A 39-year-old man with interstitial pneumonia and pulmonary hypertension exhibited lack of efficacy during treatment with nintedanib, methylprednisolone, pirfenidone, and sildenafil. Despite receiving these medications, his symptoms did not improve. He was transferred to another hospital for a lung transplant and underwent various treatments, including tracheal intubation, ventilation, and extracorporeal membrane oxygenation. His symptoms improved after tracheostomy but later developed respiratory failure, which was managed with steroid pulse therapy. [Extracted from the article]
