The following article is an element of a 4-part series on the presentation and discussion of new assessment recommendations for disability compensation in the private accident insurance. The introduction to the topic and the associated basics as well as the concept of the new draft of the assessment recommendations of the upper extremities have already been published in Die Unfallchirurgie (previously Der Unfallchirurg) on 17 February and 31 August 2022 [2, 3]. The topic of the third part published here is the assessment recommendations for disability in the lower extremities within the compensation scheme.Der nachfolgende Artikel ist Bestandteil der vierteiligen Serie zu Präsentation und Diskussion von neuen Bemessungsempfehlungen in der Privaten Unfallversicherung. Die Einführung in das Thema und die zugehörigen Grundlagen sowie das Konzept des Neuentwurfes der Bemessungsempfehlungen an den oberen Extremitäten sind bereits am 17.02.2022 und 31.08.2022 in Die Unfallchirurgie (vormals Der Unfallchirurg) [1, 2] veröffentlicht worden. 5. Minderung der Erwerbsfähigkeit (MdE) in der gesetzlichen Unfallversicherung

Die Expertengruppe unter der Schirmherrschaft der DGUV (Deutsche Gesetzliche Unfallversicherung) hat die MdE(Minderung der Erwerbsfahigkeit)-Werte nach Gliedmasenverlusten in einem interdisziplinaren methodischen Ansatz umfassend, systematisch und wissenschaftlich uberpruft. Grundlage der neuen Vorschlage sind die Gesundheits- bzw. Funktionsstorungen in Anlehnung an die ICF (International Classification of Functioning, Disability and Health) und die sich daraus ergebende Einteilung in vergleichbare Funktionsstorungsklassen. Untersucht wurde, von welchen Einschrankungen am Arbeitsmarkt fur Versicherte mit unfallbedingten Funktionseinschrankungen heute auszugehen ist. Dazu wurden eine rehabilitationswissenschaftliche Literaturrecherche und eine Befragung der masgeblichen arbeitswissenschaftlichen Institute der Bundesrepublik Deutschland durchgefuhrt. Im Ergebnis konnten daraus jedoch keine validen Arbeitsmarktdaten zu verschlossenen Beschaftigungsmoglichkeiten generiert werden. Um ihrem Auftrag und den Anforderungen des § 56 Abs. 2 Satz 1 SGB (Sozialgesetzbuch) VII unter den gegebenen Umstanden gerecht zu werden, hat die multiprofessionell zusammengesetzte Expertengruppe ihr vielfaltiges Fachwissen uber Funktionsstorungen und deren Auswirkungen im Erwerbsleben zugrunde gelegt und darauf aufbauend neue MdE-Eckwerte konsentiert, die jeweils differenziert hergeleitet und begrundet wurden. Die Vorschlage der Expertengruppe sollen zu einem gesellschaftlich, juristisch und sozialvertraglichen Konsens fuhren und damit auch gewahrlisten, dass die Unfallrente (MdE) auch zukunftig ihrer wichtigen Rolle als sozialem Befriedungsfaktor gerecht wird.

Unfallverletzte im Rahmen von Arbeits- und Wegeunfallen unterliegen dem besonderen Schutz durch die Leistungen der gesetzlichen Unfallversicherung (GUV), die im Sozialgesetzbuch VII geregelt sind. Der Leistungsgrundsatz ist mit der Bezeichnung „mit allen geeigneten Mitteln“ umschrieben, um Unfallverletzte schnellstmoglich ins Arbeitsleben zuruckzubringen. Eigens fur das Rehamanagement wurden von den Tragern der GUV mit dem Verletzungsartenverfahren Steuerungsmechanismen mit strukturellen Anforderungen an Behandlungseinrichtungen und Arzte etabliert. Ziele sind die direkte Zuweisung des Unfallverletzten in eine geeignete Klinik und die in jeder Hinsicht optimierte Versorgung ohne unnotige Verlegungen. An der Unfallstelle mit Inaugenscheinnahme, initialer Befragung des Verletzten, der Polizei oder von Zeugen resultiert ein erheblicher Informationsgewinn, von dem Klinik, Durchgangsarzt und Unfallversicherungstrager (sowie letztendlich der Versicherte) profitieren konnen. Die Informationen (genauer Unfallhergang, -ort, -betrieb, Versicherungsdaten und Kontextfaktoren) sind zu dokumentieren und an die aufnehmende Klinik und den Durchgangsarzt weiterzuleiten. Die Informationserfassung sollte in die standardisierte rettungsdienstliche Dokumentation ubernommen werden.

The centerpiece of a private accident insurer is the benefit promise in cases of disability, which must be determined by a doctor in due time. The insurer specifies the rates of the compensation scheme for loss or inability to function and the medical expert must then fall back on generally recognized, revised or updated assessment recommendations in order to be able to apply the given framework to the specific, individual situation of the insured person. In four steps (basics, upper and lower extremities [disability within the compensation scheme] and disability outside the compensation scheme) interdisciplinary consensus benchmarks for disability assessment are presented, which should form the basis of a uniform medical assessment of accident-related functional disorders in private accident insurance compensation.Das Herzstück des Leistungsversprechens eines privaten Unfallversicherers ist die Invaliditätsleistung, die ärztlich fristgerecht festzustellen ist. Der Versicherer gibt pauschalierte Sätze der Gliedertaxe für Verlust oder Funktionsunfähigkeit vor und der ärztliche Sachverständige muss dann auf allgemein anerkannte, überarbeitete/aktualisierte Bemessungsempfehlungen zurückgreifen können, um den vorgegebenen Rahmen auf die konkrete, individuelle Situation des Versicherten anwenden zu können. In 4 Schritten (Grundlagen, obere und untere Extremitäten [Invalidität innerhalb der Gliedertaxe] und Invalidität außerhalb der Gliedertaxe) werden fachübergreifend konsentierte Eckwerte der Invaliditätsbemessung vorgestellt, die Grundlage einer einheitlichen ärztlichen Begutachtung von unfallbedingten Funktionsstörungen in der privaten Unfallversicherung sein sollen.

Die Differenzierung einer traumatischen von einer unfallunabhangigen Schadigung des Lig. scapholunare (SLL) stellt auch weiterhin eine Herausforderung an den Gutachter dar. Der Nachweis einer traumatischen Zerreisung gelingt zuverlassig bei gesichertem, adaquatem Unfallhergang und beim Vorliegen von Begleitverletzungen (weitere Band-Kapsel-Rupturen, knocherne Verletzungen und/oder einem „bone bruise“). Schwierig gestaltet sich die Zusammenhangsbeurteilung bei unklaren Ereignisablaufen, einer damit verbundenen geringen Krafteinleitung und erst spater aufgetretenen, klinischen Symptomen, die dann im Rahmen weiterer Diagnostik die SLL-Ruptur aufzeigen. Hierbei gilt es, nach konkurrierenden Ursachen (angeborene Aplasie des SLL, verschleisbedingter Bandaufbrauch oder weitere traumatische Ereignisse) zu fahnden, die dann aber auch mit Sicherheit nachzuweisen sind. Haufigste Ursache fur die traumatische SLL-Ruptur ist der Sturz auf die Handflache des nach vorn ausgestreckten Armes.

Der kraniale knocherne Anteil der Schultergelenkpfanne, die damit fest verbundene knorpelige Gelenklippe und die dort entspringende Bizepssehne sowie die anhangenden Kapsel-Band-Strukturen weisen bestimmte wiederkehrende Schadigungskonstellationen auf, die von der medizinischen Wissenschaft als sog. SLAP-Lasionen („superior labrum anterior to posterior lesion“) systematisiert und eingehend beschrieben wurden. Diese werfen insbesondere auch im Hinblick auf die besonderen Kausalitatsanforderungen in der Unfallversicherung immer wieder die Frage auf, ob es Schadigungskonstellationen oder Hinweise in diesen gibt fur eine uberwiegend traumatische oder verschleisbedingte Verursachung. Im Ergebnis der Auswertung der Literatur ist festzustellen, dass die weit uberwiegende Anzahl der SLAP-Lasionen im Regelfall nicht ursachlich auf eine traumatische Schadigung zuruckgeht. Art und Richtung von Gewalteinwirkungen, die zu einem unfallbedingten Ursachenbeitrag fuhren konnen, werden diskutiert.

Die Voraussetzung zur Beurteilung der Rotatorenmanschette und damit zur Begutachtung ihrer Schaden liegt in der Kenntnis der Anatomie der Rotatorenmanschette (Kabelsystem) und deren Funktion. Symptomatische und auch asymptomatische Defekte in der Rotatorenmanschette sind in der alter werdenden Bevolkerung ein haufiges Phanomen. Wegen versicherungsrechtlicher Anforderungen ist der medizinische Sachverstandige aufgefordert, eine Unterscheidung zwischen texturstorungsbedingten (degenerativen) und traumatischen Schaden an der Rotatorenmanschette zu treffen. Der vorliegende Beitrag soll hierzu eine Hilfestellung bieten. Die Pathologie der Rotatoren an der Schulter unterscheidet echte, seltene Rotatorenrisse von den haufigen Rotatorendefekten, entstanden durch die altersabhangige Texturstorung und aufgrund verschiedener pathologischer Einflusse. Der traumatische Rotatorenriss kann durch direkte oder indirekte Krafteinwirkung bedingt sein. Die direkte Kraft fuhrt zu erheblichen lokalen Begleitverletzungen; bei indirekten Einwirkungen entgegen der eigenen Muskelkraft sind Begleitverletzungen an weiteren Strukturen zu erwarten.

Epiphysenschaden findet man im Wachstumsalter bei Madchen fruher, bei Jungen spater. Zu differenzieren sind fruhzeitiger (Teil-)Fugenschluss, Osteonekrosen, Epiphysenlosungen und Epiphysenfrakturen. Immer wieder kommt es bei Heranwachsenden wahrend schulsportlicher Betatigung oder in einem anderen gesetzlich unfallversicherten Zusammenhang zum Auftreten von Schaden an den Epiphysen. Die Ursache einer Epiphysenlosung kann im Auftreten eine Osteonekrose, aber auch eine verletzungsbedingte Schadigung sein. In aller Regel konnen moderne bildgebende Verfahren die Ursache eingrenzen. In der gesetzlichen Unfallversicherung stellt sich dann die Frage, ob ein versichertes auseres Ereignis wesentlich den nachgewiesenen Gesundheitsschaden verursacht hat oder ob andere unfallunabhangige Faktoren (Vorerkrankung oder Schadensanlage) fur die Entstehung allein wesentlich waren. Der medizinische Sachverstandige hat fur die hier zu treffende juristische Entscheidung die aktuelle wissenschaftliche Erkenntnislage zu berucksichtigen und die einzelnen Anknupfungstatsachen und deren Bedeutung fur den gegenstandlichen Gesundheitsschaden dem Gericht zu erlautern. Die vorliegende Arbeit soll fur den medizinischen Sachverstandigen den aktuellen medizinisch-wissenschaftlichen Erkenntnisstand zusammenfassen und ihm somit eine wichtige Argumentationshilfe im Abwagungsprozess der Ursachenzusammenhangsbetrachtung geben.

Die intraoperative 3‑D-Bildgebung hat die chirurgische Versorgung gelenkbeteiligender Frakturen merklich beeinflusst. Theoretisch gelingt es, in gleicher Sitzung unzureichende Repositionsergebnisse oder Implantatfehllagen zu korrigieren und somit unnotige sekundare Bildgebungen und Eingriffe zu vermeiden. Die Datenlage zur diagnostischen Genauigkeit des 3‑D-Scans beruht wesentlich auf heterogenen praklinischen Experimenten und muss entsprechend vorsichtig interpretiert werden. Jeder vierte 3‑D-Scan scheint trotz formal nicht bewiesener Sensitivitat und Spezifitat eine Neupositionierung von Frakturfragmenten oder Implantaten nach sich zu ziehen. Die Interaktion zwischen diagnostischer Genauigkeit und therapeutischen Konsequenzen muss systematischer exploriert werden, bevor Schlussfolgerungen uber den (Zusatz-)Nutzen der 3‑D-Bildgebung getroffen werden konnen.

The operative treatment of unstable distal radius fractures primarily aims for the anatomical reduction of the joint while addressing accompanying injuries. Anatomical reduction, stable fixation and early functional movement of the joint are the three cornerstones of modern treatment concepts of distal radius fractures. Distal radius volar locking plates play a major role in the treatment and rehabilitation of the most commonly occurring fracture in humans. This article outlines the different principles in the current design of available distal radius volar locking plates. The biomechanical aspects, anatomical findings and clinical evaluation that have influenced current design features and trends in new developments of the latest plates are emphasized. This is an ongoing process that is supported through the investigation and feedback of clinical science.

In der gesetzlichen Unfallversicherung sind medizinische Sachverstandigengutachten wesentliche Entscheidungsgrundlage fur die Anerkennung eines Versicherungsfalls und fur Entschadigungsleistungen. Die Gesamtzahl der Gutachten fur die Unfallversicherungstrager wird auf 80.000 jahrlich geschatzt. Die Gutachten fur die Unfallversicherungstrager werden im Rahmen eines sich ggf. anschliesenden Klageverfahrens auch von den Sozialgerichten uberpruft. Dabei besteht eine hohe Rechtssicherheit der Entscheidungen der gesetzlichen Unfallversicherung. Mit einer Rate von 17,6 % erfolgreichen Klageverfahren erster Instanz liegen die Unfallversicherungstrager deutlich unter den Korrekturquoten anderer Sozialleistungstrager. Dies zeugt von einem sorgfaltigen und juristisch belastbaren Umgang mit den z. T. sehr tragischen Einzelschicksalen, aufgrund deren ein Renten- oder Anerkennungsverfahren eingeleitet wurde. Um die Begutachtungsgute zu erhalten und noch weiter zu steigern, sind auf beiden Seiten (Auftraggeber und Gutachter) Masnahmen der Qualitatssicherung erforderlich. Im vorliegenden Beitrag wird ein Uber- und Ausblick uber die bisherigen und noch geplanten Ansatze zur Qualitatssicherung in der Begutachtung fur die gesetzliche Unfallversicherung gegeben.

The background, design, and (preliminary) results of the “Freiburg Study” on the short- and long-term sequelae of accidental injury and effects on mental health are presented. The study evaluates whether systematic, initial screening for patients at risk to develop accident-related mental health problems is a useful first step in delivering more specific forms of psychodiagnosis and treatment. The implementation of a screening-based algorithm for diagnosis and treatment of mental health problems and disorders following work-related accidents is being studied under routine conditions in 18 trauma departments. Results for the first phase of this study are presented. Preliminary conclusions are discussed with reference to clinical guidelines for acute traumatization and diagnosis and treatment of chronic PTSD.

Aligned, electrospun fiber scaffolds provide topographical guidance for regenerating neurons and glia after central nervous system injury. To date, no study has explored how fiber surface nanotopography affects astrocyte response to fibrous scaffolds. Astrocytes play important roles in the glial scar, the blood brain barrier, and in maintaining homeostasis in the central nervous system. In this study, electrospun poly L-lactic acid fibers were engineered with smooth, pitted, or divoted surface nanotopography. Cortical or spinal cord primary rat astrocytes were cultured on the surfaces for either 1 or 3 d to examine the astrocyte response over time. The results showed that cortical astrocytes were significantly shorter and broader on the pitted and divoted fibers compared to those on smooth fibers. However, spinal cord astrocyte morphology was not significantly altered by the surface features. These findings indicate that astrocytes from unique anatomical locations respond differently to the presence of nanotopography. Western blot results show that the differences in morphology were not associated with significant changes in glial fibrillary acidicprotein (GFAP) or vinculin in either astrocyte population, suggesting that surface pits and divots do not induce a reactive phenotype in either cortical or spinal cord astrocytes. Finally, astrocytes were co-cultured with dorsal root ganglia to determine how the surfaces affected astrocyte-mediated neurite outgrowth. Astrocytes cultured on the fibers for shorter periods of time (1 d) generally supported longer neurite outgrowth. Pitted and divoted fibers restricted spinal cord astrocyte-mediated neurite outgrowth, while smooth fibers increased 3 d spinal cord astrocyte-mediated neurite outgrowth. In total, fiber surface nanotopography can influence astrocyte elongation and influence the capability of astrocytes to direct neurites. Therefore, fiber surface characteristics should be carefully controlled to optimize astrocyte-mediated axonal regeneration.

Das Ursache-Wirkungs-Prinzip gilt es hierbei, unter den strengen Regeln des SGB (Sozialgesetzbuch) VII anzuwenden. Dieses kann deshalb in der am Ende stehenden Beurteilung von rein naturwissenschaftlichen Betrachtungen deutlich abweichen. Im vorliegenden Artikel soll die Aufgabe des Gutachters an einem praktischen Beispiel demonstriert und transparent gemacht werden.

In einer prospektiven Studie mit 82 konsekutiven Patienten bei denen eine operative Versorgung einer intraartikularen Fersenbeinfraktur erfolgte, wurde ein Qualitatsvergleich zwischen konventioneller Durchleuchtung, intraoperativer Iso-C3D- und postoperativer CT-Bildgebung angestellt. Die in drei Sektoren eingeteilte Facies articularis talaris posterior (FATP) wurde von zwei unabhangigen Untersuchern auf verbliebene Gelenkstufen und fracture gaps untersucht. Analysiert wurde, ob der Operateur die ISO-C3D-Schnittbilder richtig befundete und intraoperativ Konsequenzen daraus zog. Es fanden sich keine signifikanten Unterschiede bei der Befundungssicherheit zwischen Iso-C3D- und CT-Bildgebung bezuglich verbliebener Gelenkstufen in den Sektoren I–III, sowie detektierter fracture gaps in den Sektoren I und III. Mit der konventionellen Durchleuchtung waren die Sektoren I und II uberhaupt nicht zu beurteilen. In 6 Fallen (7,3%) wurde die Reposition intraoperativ nach einem Iso-C3D-Scan optimiert. Bei 10 Patienten wurden 12 Schraubenfehllagen (12,2% bzw. 14,6%) korrigiert. Die Ergebnisse zeigen, dass mit der intraoperativen Iso-C3D-Bildgebung eine hohe Befungssicherheit zu erreichen ist. Der Operateur erhalt relevante Zusatzinformationen, die zu einer Anderung der OP-Strategie fuhren konnen.

Fur die Dokumentation und Visualisierung komplexer anatomischer Strukturen wie Fersenbein, Becken, Wirbelsaule usw. ist der CT-Scan Goldstandard. In den Operationssalen der orthopadischen Chirurgie steht der CT-Scan leider nicht zur Verfugung, obwohl er dort sehr effektiv vor dem Wundverschluss eingesetzt werden konnte, denn sein Einsatz ist mit hohen Kosten, baulichen Veranderungen der Op-Sale, zusatzlichem Personal etc. verbunden. Die ISO-C3D-Technik ermoglicht es dem Chirurgen erstmals, wahrend einer Operation innerhalb eines akzeptablen Zeitrahmens, zu angemessenen Kosten und mit Hilfe eines ihm vertrauten Instruments (ein modifizierter C-Bogen) multiplanare Rekonstruktionen zu produzieren. Seit Februar 2001 steht uns im Unfallkrankenhaus Berlin ein ISO C3D zur Verfugung, und es wurden bereits 442 chirurgische Scans, inklusive Fersenbein, Halswirbelsaule, Huftgelenkspfanne etc., durchgefuhrt. Aufgrund der intraoperativen Bildgebung ergab sich in Bezug auf unbefriedigende Fraktur- und Implantatpositionierungen insgesamt eine Revisionsrate von 7,3%. Die ISO-C3D-Technik ist der erste Schritt in die Zukunft der neuen Moglichkeiten zur Knochenvisualisierung. Im nachsten Schritt wird der Scan-Bereich erweitert und die Bildauflosung erhoht werden, die Hochkontrastbilder werden durch Bilder mit deutlich sichtbaren Weichgewebsstrukturen, ahnlich den modernen CT-Schnitten, ersetzt werden.

Posttraumatische Fehlstellungen der Wirbelsaule sind haufig Folge einer inadaquaten Stabilitatsbeurteilung und eines daraus resultierenden unzureichenden Behandlungskonzepts nach einem frischen Trauma. Sie konnen auch aus einer regelrechten Behandlung resultieren, insbesondere wenn die knochernen Heilungsprozesse deutlich langsamer als erwartet voranschreiten und ein Implantatversagen auftritt, wobei die notwendige Kapazitat der Belastungsaufnahme des Knochens noch nicht ausreicht. Verletzungen der Wirbelsaule konnen zu einer veranderten Statik fuhren und durch Fehlbelastungen kurz- und langfristige Probleme hervorrufen. Die Behandlung von Folgezustanden nach Frakturen der Brust- und Lendenwirbelsaule stellt eine besondere Herausforderung dar. Es folgt die Beschreibung von biomechanischen Grundprinzipien, der Technik der Bestimmung von Fehlstellungen, von Grundzugen der Indikationsstellung zur Korrektur und von Moglichkeiten der operativen Techniken.

Introduction Prior evidence suggests that men and women might be differentially susceptible to distinct types of childhood adversity (CA), but research on gender-specific associations between CA subtypes and psychiatric symptoms is limited. Objectives To test the gender-specific associations of CA subtypes and psychiatric symptoms in the general population. Methods Data from 791 twins and siblings from the TwinssCan project were used. Psychopathology and CA exposure were assessed using the Symptom Checklist-90 Revised (SCL-90) and the Childhood Trauma Questionnaire (CTQ), respectively. The associations between the total CTQ scores and SCL-90 scores (i.e. total SCL-90, psychoticism, paranoid ideation, anxiety, depression, somatization, obsessive-compulsive, interpersonal sensitivity, hostility, and phobic anxiety) were tested in men and women separately. The associations between the five CA subtypes (i.e. physical abuse, emotional abuse, sexual abuse, physical neglect, and emotional neglect) and total SCL-90 were tested in a mutually adjusted model. As exploratory analyses, the associations between all CA subtypes and the nine SCL-90 subdomain scores were similarly tested. The regression coefficients between men and women were compared using Chow’s test. All models were adjusted for age and family structure. Results Total CTQ was significantly associated with total SCL-90 in men (B = 0.013, SE = 0.003, P < .001) and women (B = 0.011, SE = 0.002, P < .001). The associations with the nine symptom domains were also significant in both genders (P < .001). No significant gender differences in the regression coefficients of total CTQ were detected. The analyses of CA subtypes showed a significant association between emotional abuse and total SCL-90 in women (B = 0.173, SE = 0.030, P < .001) and men (B = 0.080, SE = 0.035, P = .023), but the association was significantly stronger in women (ꭓ2(1) = 4.10, P = .043). The association of sexual abuse and total SCL-90 was only significant in women (B = 0.217, SE = 0.053, P < .001). The associations of emotional neglect (B = 0.061, SE = 0.027, P = .026) and physical neglect (B = 0.167, SE = 0.043, P < .001) with total SCL-90 were only significant in men. The explorative analyses of SCL-90 subdomains revealed significant associations of emotional abuse with all nine symptom domains and of sexual abuse with seven symptom domains in women. Significant associations of physical neglect with six symptom domains and of emotional neglect with depression were also detected in men. No other significant associations between CT subtypes and total SCL-90 or symptom domain scores were observed in men and women. Conclusions CA exposure was associated with diverse psychopathology similarly in both genders. However, women are more sensitive to abuse, but men are more sensitive to neglect. In addition to stability and operational safety, leak tightness is the permanent functional objective of wastewater pipes. Tests to determine the tightness of wastewater pipes can in some cases produce results that are worthy of discussion. Therefore, laboratory tests were carried out by 29 specialist contractors to obtain results on the quality of leak tests and visual inspections of connection pipes. The results showed that different test errors can be observed for leak test methods (air overpressure, air underpressure and water pressure). However, only in the case of the water pressure tests did the observed test errors occasionally lead to incorrect test results, i.e. the ‘leaking pipe’ was tested as ‘test passed (tight)’. The investigations into the accuracy (trueness and precision) of the test methods showed that all test methods examined were sufficiently accurate to determine the tightness of the connection pipes. In general, correct test results were achieved if the expert testers did not make any serious test errors and the test equipment used functioned properly. In contrast, the investigations on the quality of visual inspection showed that the procedure is not sufficiently reliable to fulfil all normative requirements regarding damage detection and naming as well as damage classification. Tests to determine the tightness of wastewater pipes can in some cases produce results that are worthy of discussion. Therefore, testing procedures for real-scale sewage pipes used for house connections were evaluated and data were statistically analysed. The results of the investigation showed that leaky pipes are detected with a very high degree of reliability by all leak test methods. The test methods are also robust against errors by expert testers and deviations from the test specifications. In contrast, tight pipelines can also be incorrectly classified as ‘leaking’ (test failed) to a significant extent during leak tests. Even for the more reliable test methods, i.e. air overpressure, air underpressure and water with low test pressure, a tight pipe is incorrectly classified as leaking (false positive) in one out of 10 cases (10%). The highest false-positive rate was 20% for water with high test pressure. In addition to the leak test methods, the quality of the visual inspection was also analysed. Here it was found that visual inspection is not sufficiently reliable for determining the tightness of pipelines above the groundwater level. Error rates of approximately 50% were found for the detection of tight and leaky pipelines. The uncertainty associated with the determination of load parameters, which is a key step in the design of wastewater treatment plants (WWTPs), was investigated on the basis of data sets from 58 WWTPs. A further analysed aspect was the organic load variations associated with variable sewage temperatures. Data from 26 WWTPs with a high inflow sampling frequency was used to simulate scenarios to investigate the effect of lower sampling frequencies through a Monte Carlo approach. The calculation of 85-percentile values for chemical oxygen demand (COD) loadings based on only 26 samples per year is associated with a variability of up to ±18%. Approximately 90 samples per year will be necessary to reduce this uncertainty for estimation of COD loadings below 10%. Hence, a low sampling frequency can potentially lead to under- or overestimation of design parameters. Through an analogous approach, it was possible to identify uncertainties of ±11% in COD loading when weekly average data was used with four samples per week. Finally, a tendency to lower COD input loads with increasing temperatures was identified, with a reduction of about 1% of the average loading per degree Celsius. Introduction Group-level studies showed cross-sectional and prospective between-person associations between circadian rest-activity rhythms (RAR), physical activity (PA), sleep, and depressive symptoms. However, whether these associations replicate at the within-person level remains unclear. Therefore, it is clinically relevant to investigate these associations within persons and study whether changes in depressive symptoms are related to changes in circadian rhythm and sleep variables. Objectives To identify changes in circadian rhythm elements in proximity to a transition in depressive symptoms, whether changes are less frequent in individuals without compared to those with transitions, and whether there are individual differences in the direction of change of circadian rhythm variables. Methods Data of remitted individuals tapering antidepressants were used: 12 with and 14 without a transition in depressive symptoms. RAR, PA, and sleep variables were calculated as predictors from four months of actigraphy data. Transitions in depressive symptoms were based on weekly SCL-90 scores and evaluation interviews. Kernel Change Point analyses were used to detect change points (CPs) and CP timing in circadian rhythm variables for each individual separately. Results In 67% of individuals with depressive symptoms transitions, CPs were identified in proximity to symptom transitions. CPs were detected less frequently in the no-transition group with 7 CPs in 14 individuals, compared to transition groups with 10 CPs in 12 individuals. For several RAR and sleep variables, consistent changes were detected in expected directions. Conclusions Circadian rhythm variables provide potentially clinically relevant information although their patterns around transitions are highly person-specific. Future research is needed to disentangle which variables are predictive for which patients. Disclosure No significant relationships.

Computed tomography (CT) scanning is the gold standard for displaying and visualizing complex anatomic structures such as the calcaneus, pelvis, and spine, etc. In orthopedic surgery CT scanning is unfortunately not available in the OR where it would be highly effective if scans could be done prior to wound closure. The reasons are high costs, need for constructional changes in the OR, and additional staff (radiologic technologist), etc. For the first time the ISO C(3D) enables the surgeon to generate multiplanar reconstruction (MPR) pictures during surgery within an acceptable time frame at a reasonable price with a familiar instrument (a modified C-arm). Since February 2001 we have been able to use the ISO C(3D) in our hospital and meanwhile we have scanned 442 surgical sites including the calcaneus, cervical spine, and acetabulum, etc. The intraoperative scans revealed fracture gaps and steps, unsatisfactory fragment alignment, or incorrectly positioned implants, leading to an overall revision rate of 7.3%The ISO C(3D) is a first step towards a new future for bone visualization. The next steps will bring an increase in scan area and resolution, and the high-contrast pictures will be replaced by ones very similar to modern CT slices showing certain soft tissue structures.

With 44.9 % trauma is the main cause of death in men under 40 years. From September 3(rd) 1997 to June 30(th) 1998 174 patients with severe injuries or polytrauma were treated in our hospital. The mean ISS was 29 (18-75). 15 (8.75 %) patients died within the first 24 hours. The mean age was 34.5 (2-85) years. The mean ISS of these patients was 48 (25-75). In a retrospective study we analyzed the pattern of injury as well as the preclinical and clinical management. The fact that orthopedic surgery still has got a poor position within the patient's satisfaction-scale and that 15 of 174 analyzed patients had to die within the first 24 hours points out the importance of internal and external quality improvement measurements.

Fractures of the humeral head are still challenging to decisions of indications, surgical skills and physiotherapy. 3- and 4-part-fractures of the humeral head in younger adults are still dominated by reconstructive osteosynthesis. New developments in osteosynthesis materials allow the surgeon to make use of minimal invasive techniques more often. Treatment of elderly patients with these injuries has changed within the last years. When it is obvious that the goal of a stable osteosynthesis can not be reached, shoulder prosthesis has become a primary treatment in elderly patients. New designs in shoulder prosthesis seem to lead to better functional results.

We report the case of a 62-year-old woman with marked valgus and limitation of extension of the right knee joint as the result of a supracondylar fracture of the femur, originally treated by internal fixation with a dynamic condylar screw. Two years later, revision osteotomy with retrograde femoral nailing was carried out to achieve variation and improve extension. Within a week of operation the patient's right leg was fully weight bearing with normal axial positioning. A retrograde locking nail provides satisfactory and stable internal fixation in cases of revision.

We previously reported that chronic exposure of male rats to morphine markedly increases the concentration of corticosteroid-binding globulin (CBG) in blood. This in turn appears to greatly reduce the amount of corticosterone available to intracellular receptors. In the study reported here, we found that in contrast to the effect in males, morphine has no apparent effect on CBG in females. This pronounced sex difference does not appear to be attributable to differences in morphine pharmacokinetics, short-term actions of gonadal hormones in adulthood or sex differences in CBG or corticosterone levels. In any case, it is evident that morphine does not decrease the level of physiologically active corticosterone through CBG in females as it appears to do in males. On the other hand, we also found a distinct sex difference with regard to the effects of morphine on corticosterone. Chronic exposure to morphine had no apparent effect on corticosterone levels in males but resulted in markedly lower levels in females. Thus, morphine appears to cause a deficit in physiologically active corticosterone in both sexes but by different mechanisms.

Although it appears that corticosterone may play an important role in determining vulnerability to drugs of abuse, few studies have examined drug effects on factors that affect corticosterone efficacy. Thus, studies were carried out to assess the effects of morphine on corticosteroid-binding globulin (CBG), the major glucocorticoid binder in blood. Since CBG-bound hormone is thought to be physiologically inactive, changes in CBG levels could affect corticosterone action independently of hormone levels per se. We found that morphine caused a marked naltrexone-preventable increase (approximately 160%) in CBG in adult male rats. Elevated levels were seen by three days and were maximal at seven days after morphine pellet (75 mg) implantation. CBG levels remained elevated while morphine was detectable in blood and returned toward normal as the drug cleared from the system. A single morphine pellet was sufficient to induce a marked increase in the concentration of CBG and two or more pellets caused maximal upregulation. Baseline and stress levels of total corticosterone (bound and unbound) were normal after chronic exposure to morphine. However, due to the elevated level of CBG, the amount of free, physiologically active hormone was dramatically reduced. These results suggest that morphine may exert potent effects on corticosterone action that are not revealed by measurement of corticosterone alone. Furthermore, the increase in CBG resulting from chronic exposure to morphine might contribute to the perpetuation of drug use and to adverse effects of drug exposure by impairing normal functions of corticosterone.

Digitonin is widely used for extracting active neurotransmitter receptors from membranes. However, its low critical micellar concentration has made its removal from samples problematic. Here we report that digitonin can be efficiently removed (90%) from solution using Extracti-Gel D, a detergent-absorbing matrix. Active kappa 1 opioid receptors solubilized from brain survive Extracti-Gel D chromatography with a recovery of 50-55% and 25% dilution by added volume. The loss of receptor and the dilution, however, are compensated for to a large extent by the disinhibition of binding that results from the removal of digitonin. Extracti-Gel D chromatography had little or no effect on the apparent equilibrium dissociation constant for [3H]U-69,593 binding to the kappa 1 receptor. We conclude that Extracti-Gel D column chromatography is a simple, highly efficient and practical method for markedly reducing the concentration of digitonin in biological samples. Application of the procedure should allow characterization of digitonin-solubilized receptors with minimal complications from bound digitonin and extend the usefulness of digitonin to studies going beyond the initial stages of receptor purification.

Introduction In many countries, depressed individuals often first visit primary care settings for consultation, but a considerable number of clinically depressed patients remains unidentified. Introducing additional screening tools may facilitate the diagnostic process. Objectives This study aims to examine whether Experience Sampling Method (ESM)-based measures of depressive affect and behaviors can discriminate depressed from non-depressed individuals. In addition, the added value of actigraphy-based measures was examined. Methods We used data from two samples to develop and validate prediction models. The development dataset included 14 days of ESM and continuous actigraphy of currently depressed (n=43) and non-depressed individuals (n=82). The validation dataset included 30 days of ESM and continuous actigraphy of currently depressed (n=27) and non-depressed individuals (n=27). Backward stepwise logistic regression analyses were applied to build the prediction models. The performance of the models was assessed with the goodness of fit indices, calibration curves, and discriminative ability (AUC, the area under the receiver operating characteristic curve). Results In the development dataset, the discriminative ability was good for the actigraphy model (AUC=0.790) and excellent for the ESM (AUC=0.991) and combined-domains model (AUC=0.993). In the validation dataset, the discriminative ability was reasonable for the actigraphy model (AUC=0.648) and excellent for the ESM (AUC=0.891) and combined-domains model (AUC=0.892). Conclusions ESM is a good diagnostic predictor and is easy to calculate, and, therefore, holds promise for implementation in clinical practice. Actigraphy shows no added value to ESM as a diagnostic predictor, but might still be useful when active monitoring with ESM is not feasible. Disclosure No significant relationships.
