Objective To explore the effect of stepped rehabilitation nursing in elderly patients with a- cute exacerbation of chronic obstructive pulmonary disease (AECOPD). Methods A total of 74 elderly patients with AECOPD admitted to the hospital from January 2022 to January 2023 were divided into two groups by random number table method, 37 cases in each group. The control group was given routine nursing. while the observation group was given routine nursing combined with stepped rehabilitation nursing. The lung function, motor function, degree of dyspnea, daily living ability, psychological status and complications were compared between the two groups. Results After eight weeks of discharge, the forced expiratory volume in the first second (FEV₁) and FEV₁/forced vital capacity of the two groups were higher than those at admission, and the observation group was higher than the control group, the differences were statistically significant (P<0.05). The modified medical research council dyspnea scale scores of the two groups were lower than those at admission, and the observation group was lower than the control group. The six min walking distance and Barthel index scores of the two groups were higher than those at admission, and the observation group was higher than the control group. The differences were statistically significant (P<0.05). The scores of self- rating anxiety scale and self-rating depression scale in the two groups were lower than those at admission, and the scores in the observation group were lower than those in the control group, the differences were statistically significant (P<0.05). There was no significant difference in the incidence of complications between the two groups at hospitalization and follow-up after discharge (P>0.05). Conclusion Stepped rehabilitation nursing for elderly patients with AECOPD can improve lung function. improve daily living ability and exercise ability, and alleviate negative emotions. Objective: To observe the effects of inspiratory muscle training under low intensity [20% maximum inspiratory pres- sure (MIP)] and medium intensity (40% MIP) threshold load on diaphragm function, lung function and quality of life in patients with chronic obstructive pulmonary disease (COPD). Methods: 120 COPD patients who were admitted to Heze Hospital of Shandong Provin- cial Hospital from April 2020 to May 2023 were divided into control group, low intensity group and medium intensity group according to the random number table method, with 40 cases in each group. Control group was given routine treatment; On the basis of control group, the low intensity group used 20% MIP as the initial load of inspiratory muscle training; Medium intensity group used 40% MIP as the ini- tial load of inspiratory muscle training. The lung function [forced vital capacity (FVC), MIP, forced expiratory volume in the first second (FEV1), peak expiratory flow (PEF), maximum ventilation volume per minute (MVV)], diaphragm function [diaphragmatic displacement (DE), deep inspiratory diaphragm thickness (DTei) and quiet end-expiratory diaphragm thickness (DTee), diaphragmatic thickness frac- tion (DTF)] and quality of life [COPD quality of life scale (COPD-QOL), St George's respiratory disease questionnaire (SGRQ)] were compared among three groups. Results: After treatment, MIP, FVC, FEV₁, MVV and PEF in control group, low intensity group and medi- um intensity group increased in turn (P<0.05). After treatment, DE, DTei and DTF in control group, low intensity group and medium intensity group increased in turn (P<0.05). After treatment, DTee in control group, low intensity group and medium intensity group decreased in turn (P<0.05). After treatment, the scores of COPD-QOL and SGRQ in control group, low intensity group and medium intensity group decreased in turn (P<0.05). Conclusion: Inspiratory muscle training can effectively improve lung function, diaphragm function and quality of life in COPD patients, and the effect of moderate intensity threshold load is the best. To observe the effects of ambroxol hydrochloride combined with respiratory muscle strength training on lung function, respiratory muscle strength and serum levels of high mobility group protein 1 (HMGB1), transforming growth factor-ẞ1 (TGF-β1) and serum amyloid A (SAA) in elderly patients with stable stage chronic obstructive pulmonary disease (COPD). Methods: 123 elderly patients with stable stage COPD admitted to our hospital from March 2021 to April 2023 were divided into control group (ambroxol hydrochloride treatment, 61 cases) and observation group (ambroxol hydrochloride combined with respiratory muscle strength training, 62 cases) by random number table method. The efficacy, lung function, respiratory muscle strength, serum HMGB1, TGF-β1, SAA levels and adverse reactions were compared between two groups. Results: Compared with the control group of 78.69%, the clinical total effective rate of the observation group further increased by 93.55% (P<0.05). Compared with control group, the forced expiratory volume in the first second (FEV1), forced vital capacity (FVC), FEV1/FVC, maximum expiratory pressure (MEP) and maximum inspiratory pressure (MIP) were higher in observation group at 2 months after intervention, and HMGB1, TGF-β1, and SAA were lower (P<0.05). There was no difference in the incidence of adverse reactions between two groups (P>0.05). Conclusion: Ambroxol hydrochloride combined with respiratory muscle strength training in the treatment of elderly patients with stable stage COPD, which can effectively improve the clinical treatment effect, improve the lung function and respiratory muscle strength of patients, and regulate the levels of serum HMGB1, TGF-β1 and SAA without increasing the incidence of adverse reactions. Objective: This study was to investigate the correlation between airway inflammation and lung function impairment in patients with bronchiectasis with different FEV, and to analyze the influencing factors of lung function impairment. Methods: 80 patients with bronchiectasis admitted to the respiratory department of our hospital from May 2022 to January 2024 were selected as research objects. General data of each group were analyzed, and lung function and airway inflammatory cytokine levels were compared in each group. Pearson linear analysis was used to analyze the relationship between lung function and airway inflammation in patients with bronchiectasis, and multi-factor Logisitic regression was used to analyze the influencing factors of lung function impairment in patients with bronchiectasis. Results: The FVC, FEV1/FVC and DLCO evaluation of the three groups were included in the study and the difference among the groups was compared. The results showed that the FVC, FEV1/FVC and FVC of the high FEV, group and the medium FEV, group were higher than those of the low FEV, group. FVC, FEV1/FVC and FVC in high FEV, group were higher than those in medium FEV, group (P<0.05). The levels of ESR, CRP, WBC, NEUT, PCT, IL-4, IL-6, IL-10 and TNF-a in the three groups were included in the study and the differences between the groups were compared. The levels of ESR, CRP, WBC, NEUT, PCT, IL-4, IL-6 and TNF-a in high and medium FEV, groups were lower than those in low FEV, group, and the levels of IL-10 were higher than those in low FEV₁ group (P<0.05). The levels of ESR, CRP, WBC, NEUT, PCT, IL-4, IL-6 and TNF-a in high FEV₁ group were lower than those in medium FEV₁ group, and the levels of IL-10 were higher than those in medium FEV, group(P<0.05). The results of Pearson linear analysis showed that FEV₁, FVC, FEV1/FVC and DLCO were positively correlated with airway inflammatory factors ESR, CRP, WBC, NEUT, PCT, IL-4, IL-6 and TNF-a, and negatively correlated with IL-10 in patients with bronchiectasis (P<0.05). Independent variables were set as the indicators with differences in comparison in the above data, including FVC, FEV1/FVC, DLCO, ESR, CRP, WBC, NEUT, PCT, IL-4, IL-6, IL-10, and TNF-a. The dependent variable was impaired lung function in patients with bronchiectasis. Multivariate Logisitic regression analysis was performed, and the results showed that The influential factors of impaired lung function in patients with bronchiectasis are FVC, FEV1/FVC, DLCO, ESR, CRP, WBC, NEUT, PCT, IL-4, IL-6, IL-10, TNF-α. Conclusion: Airway inflammation increases with the increase of FEV, level in patients with bronchiectasis, which is closely related to lung function and is an influential factor of lung function impairment. Objective To study the effect of advanced early rehabilitation training on successful weaning and lung function of patients with mechanical ventilation in respiratory intensive care unit (RICU). Methods A total of 106 RICU patients with mechanical ventilation admitted to the hospital from January 2020 to December 2021 were selected as the study objects. They were randomly divided into group A (n=53) and group B (n=53) by computer numbering method. Group B received routine nursing, while group A received advanced early rehabilitation training on the basis of group B. The ventilator dependence rate, successful weaning probability, mechanical ventilation time, hospital stay of RICU, respiratory mechanics index, oxygenation index and functional independence were compared between the two groups. Results The rate of ventilator dependence in group A[33.96% (18/53)] was lower than that in group B [58. 49% (31/53) ], the probability of successful weaning was higher than that in group B. and the mechanical ventilation time and hospital stay of RICU were shorter than those in group B, with statistical significance (P<0.05). The mean airway pressure and peak airway pressure when leaving RICU in both groups were lower than those when entering ICU. and the dynamic compliance, exercise and cognitive scores were significantly higher than those when entering RICU, and all oxygenation indexes were significantly improved. Moreover, the mean airway pressure and peak airway pressure of the patients in group A were significantly lower than those in group B, and the dynamic compliance, exercise and cognitive scores were significantly higher than those in group B, and the improvement degree of various oxygenation indexes was significantly better than that in group B, with statistical significance (P<0.05). Conclusion Advanced early rehabilitation training can improve the probability of successful weaning in patients with RICU mechanical ventilation, and is beneficial to the improvement of lung function and functional independence. Objective: To explore the effect of acupoint application combined with respiratory training in patients with stable chronic obstructive pulmonary disease (COPD). Method: 120 cases of stable COPD patients admitted to our hospital from December2021 to 2023.12 were selected and divided into Joint group and matched group, 60 cases each. The control group was given conventional western medicine symptomatic treatment, and the Joint group was based on the matched group, with acupoint plastering with respiratory training. Compare the related indexes. Result: The total response rate was higher than the Joint group (P＜0.05); the lung function index level increased post-treatment, and the Joint group was higher (P＜0.05); CD4+, CD3+, CD4+/CD8+and CD8+compared with the Joint group (P＜0.05); the CAT score was lower in both patients in the treatment groups, and the Joint group was lower than the matched group (P＜0.05). Objective: To study the relationship between serum long chain non coding RNA-human plasmacytoma transformation and migration gene 1 (LncRNA-PVT1), microribonucleic acid-497-5p (miR-497-5p) expression and lung function and prognosis in patients with chronic obstructive pulmonary disease (COPD) with lung cancer. Methods: 116 patients with COPD combined with lung cancer underwent concurrent surgical treatment admitted to Linyi Cancer Hospital from March 2018 to March 2020 were selected as the observation group, while 50 patients with simple COPD during the same period were included in the control group. The relative expression levels of serum LncRNA-PVT1 and miR-497-5p and lung function indexes [forced expiratory volume in the first second (FEV1), forced vital capacity (FVC), FEV1/FVC, percentage of forced expiratory volume in the first second to predicted value (FEV1%pred)] were compared between two groups. The correlation between serum LncRNA-PVT1, miR-497-5p expression and lung function indexes in patients with COPD with lung cancer were analyzed by Pearson test. Patients with COPD with lung cancer were followed up for 3 years after operation, the prognostic factors of patients with COPD with lung cancer were analyzed by multivariate Cox regression model. 3 years overall survival rate of patients with low expression and high expression of LncRNA-PVT1 and miR-497-5p were analyzed by drawn Kaplan-Meier survival curve. Results: Compared with control group, the expression level of serum LncRNA-PVT1 in observation group was significantly increased (P<0.05), and the expression level of miR-497-5p was significantly decreased (P<0.05). Compared with control group, FEV1, FEV1% pred and FEV1/FVC in observation group were significantly lower(P<0.05). Pearson test results showed that, serum LncRNA-PVT1 was negatively correlated with FEV1, FEV1% pred and FEV1/FVC in patients with COPD with lung cancer (P<0.05); serum miR-497-5p was positively correlated with FEV1, FEV1% pred and FEV1/FVC(P<0.05). The Kplan-Meier survival curve showed that, the 3 years overall survival rate in LncRNA-PVT1 high expression group was 29.23%, which was significantly lower than 47.73% in LncRNA-PVT1 low expression group (P<0.05). 3 years overall survival rate in miR-497-5p high expression group was 46.77%, which was significantly higher than 23.40% in miR-497-5p low expression group (P<0.05). Multivariate Cox regression analysis showed that, lymph node metastasis, TNM stage IIIa, poor differentiation of tumor tissue, high expression of LncRNA-PVT1 and low expression of miR-497-5p were risk factors for prognosis of patients with COPD with lung cancer (P<0.05). Conclusion: LncRNA-PVT1 is highly expresse and miR-497-5p is lowly expresse in the serum of patients with COPD with lung cancer, the expression level is relate to lung function indexes, and the higher the expression level of LncRNA-PVT1 and the lower the expression level of miR-497-5p, the lower the 3 years overall survival rate. Ischemic stroke is a cerebrovascular disease with high incidence and disability rate. After ischemic stroke, different parts of the nervous system injury may lead to different degrees of diaphragmatic dysfunction, which not only affects the respiratory function and physical stability of patients, but also restricts the overall rehabilitation process of patients. According to the characteristics of diaphragmatic dysfunction at different stages after ischemic stroke, selecting appropriate rehabilitation treatment techniques will effectively improve diaphragmatic function, enhance lung function reserve and physical stability, and provide a basis for motor function rehabilitation. Background Preserved Ratio Impired Spirometry (PRISm) refers to the non-obstructive pulmonary function abnormalities with a decrease forced expiratory volume in one second (FEV1) while the FEV1/forced vital capacity (FVC) remains constant. The PRISm population may be one of the pre-chronic obstructive pulmonary disease (COPD) populations. However, the incidence, characteristics and risk factors of PRISm in the physical examination population in China are still unknown. Objective To analyze the characteristics and influencing factors of PRISm in physical examination population. Methods From January 2017 to December 2019, a total of 970 patients received physical examination in the Third Medical Center of Chinese PLA General Hospital were selected. The baseline data and physical examination results were collected, pulmonary function was measured for vital capacity (VC), FVC, FEV1, forced expiratory volume in six second (FEV6), FEV1/FVC, peak expiratory flow (PEF), FEF25, FEF50 and FEF75, the relevant indexes were expressed as "%pre" as a percentage of the expected value. The included subjects were divided into the airflow obstruction group with FEV1/FVC60 years (OR=0.951, 95%CI=0.602-1.504, P=0.002), smoking history (OR=2.201, 95%CI=1.519-3.187, P60 years old, with the history of smoking and hypertension. Early intervention may alleviate the progression of COPD.

BackgroundWelders' exposure to welding fumes with multiple metals leads to decreased pulmonary function. Previous studies have focused on single metal exposure, while giving little attention to the impact of metal mixtures. ObjectiveTo assess the association between metal levels in urine and blood of welders and pulmonary function indicators, and to identify key metals for occupational health risk assessment. MethodsQuestionnaire surveys, lung function tests, urine and blood sampling were conducted among welders and control workers in a shipyard in Shanghai. Inductively coupled plasma mass spectrometry (ICP-MS) was used to detect the concentrations of 12 metals such as vanadium, chromium, and manganese in urine and blood. Spearman correlation was applied to analyze the correlations between the metals in urine and blood. Multiple linear regression, weighted quantile sum (WQS) and Bayesian kernel machine regression (BKMR) were used to analyze the relationships between mixed metal exposure and pulmonary function parameters, such as forced vital capacity (FVC), forced vital capacity as a percentage of predicted value (FVC%), forced expiratory volume in the first second (FEV1), forced expiratory volume in the first second as a percentage of predicted value (FEV1%), and forced expiratory volume in the first second/forced vital capacity (FEV1/FVC). ResultsThis study enrolled 445 subjects, including 322 welders (72.36%) and 123 controls (27.64%). The mean age of the 445 participants was (37.64±8.80) years, and 87.19% participants were male. Objective: To analyze the correlation between serum progranulin (PGRN), secreted frizzled related protein1 (SFRP1),chemokine cytokines ligand 26 (CCL26), lung function, and airway inflammation in children with acute exacerbation of bronchial asthma. Methods: 129 children with acute exacerbation of bronchial asthma (acute group) who were treated in our hospital from July 2019 to December 2022 were selected. According to lung function examination and combined with clinical characteristics, the children were divided into mild group (n=51), moderate group (n=42) and severe group (n=36) . And another 60 children with clinical remission of bronchial asthma (control group) who came to our hospital for treatment during the same period were selected. The serum PGRN,SFRP1, CCL26, lung function and airway inflammation of children with bronchial asthma in the control group and each subgroup of acute group were compared. Pearson correlation was used to analyze the correlation between serum PGRN, SFRP1, CCL26 and lung function indexes and airway inflammation indexes. Results: The PGRN in the severe group, moderate group and mild group was lower than that in the control group, SFRP1 and CCL26 were higher than that in the control group (P＜0.05) . With the increase of the severity of the disease, the PGRN of children with bronchial asthma decreased continuously, and SFRP1 and CCL26 increased continuously (P＜0.05) . Forced vital capacity (FVC), forced expiratory volume in the first second (FEV1) and maximum peak expiratory flow rate (PEF) in the severe group, moderate group and mild group were lower than those in the control group (P＜0.05) . FVC, FEV1and PEF in children with bronchial asthma decreased continuously with the increase of the severity of the disease (P＜0.05) . The interleukin-8 (IL-8), tumor necrosis factor-α (TNF-α) and interleukin-17 (IL-17) in the severe group, moderate group and mild group were higher than those in the control group (P＜0.05) . The IL-8, TNF-α and IL-17 in children with bronchial asthma increased continuously with the increase of the severity of the disease (P＜0.05) . The results of Pearson correlation analysis showed that PGRN was positively correlated with lung function indexes, and negatively correlated with airway inflammation indexes (P＜0.05) . SFRP1 and CCL26 were negatively correlated with lung function indexes, and positively correlated with airway inflammation indexes (P＜0.05) . Conclusion: Serum PGRN is decreased abnormally in children with acute exacerbation of bronchial asthma, and SFRP1 and CCL26 are increased abnormally, which are involved in the severity of the disease, and they are correlated with lung function and airway inflammation. Objective To investigate the effect of using pulmonary surfactant (PS) during different time periods of mechanical ventilation in the treatment of neonatal respiratory distress syndrome (NRDS) on blood gas and lung function indicators in children. Methods A total of 80 children with NRDS who received treatment at Neonatal Intensive Care Unit, Nanjing Jiangbei Hospital from February 2021 to February 2023 were selected and divided into observation group A, observation group B, observation group C and control group randomly (n=20, each). All the children received routine mechanical ventilation and comprehensive treatment for 72 hours. In the observation group A, B and C, the same dose of PS (70 mg/kg) was slowly injected into the intake pipe intubation within 0.5 h, 2-4 h and 4-6 h after mechanical ventilation, respectively. The blood gas indexes, lung function indexes, clinical efficacy and complications of the four groups before mechanical ventilation and 6 hours after PS treatment were compared. Results The improvement of blood gas indexes (PaO2, PaCO2, pH value) in the observation group A, B and C were significantly better than those in the control group at 6 h after PS (P＜0.05). The pulmonary function indexes, spontaneous breathing times, lung compliance and blood oxygen saturation recovery time of children in observation group A, B and C after using PS were prolonged in turn, but they were significantly better than those in control group (P＜0.05). The total efficiency of the observation group was significantly higher than that of the control group (98.33% vs 80.00%, χ2=5.760, P=0.016). The incidence of complications in the observation group during treatment was significantly lower than that in the control group (3.33% vs 25.00%, χ2=6.314, P=0.012). Conclusion The combination of mechanical ventilation and PS treatment can improve the prognosis of children with NRDS. The earlier the use of PS, the better the effect.

To investigate the correlation between COPD assessment test (CAT) scores and lung function as well asprognos is of patients of chronic obstructive pulmonary disease (COPD) with different phenotypes. A total of 361 patients with COPD were involved in this study. Their clinical data, CAT scores, lung function test results, extrapulmonary comorbidities and intrapulmonary complications were collected. Then, these patients were divided into emphysema group (n=200) and the bronchitis group(n=161) according to their clinical phenotype. The correlation was analyzed between CAT scores, lung function, and prognosis in emphysema group and bronchitis group. The CAT score of the emphysema group was higher than that of the bronchitis group(P<0.05). The percentage of forced expiratory volume in one second (FEV1 ) of the expected value (FEV1% ) and FEV1/forced vital capacity (FVC) were lower than those in the bronchitis group (P<0.05). The inspiratory capacity/total lung capacity (IC/TLC) was lower than that of the bronchitis group, and the residual volume/total lung capacity (RV/TLC) was higher than that of the bronchitis group (P<0.05). The incidence rates of pulmonary interstitial disease and pulmonary hypertension in the emphysema group were higher than those in the bronchitis group (P<0.05). The CAT scores of both groups were negatively correlated with FEV1%, FEV1/FVC and IC/TLC (P<0.05),and positively correlated with RV/TLC (P<0.05). The correlation of emphysema parameters was higher (P<0.05). There were statistically significant differences in the incidence rates of pulmonary interstitial disease and pulmonary hypertension between patients with different CAT scores in emphysema group (P<0.05). There was a statistically significant difference in the incidence of pulmonary hypertension between patients with different CAT scores in the bronchitis group (P<0.05). As the CAT score increased, the incidence rates of pulmonary interstitial disease and pulmonary hypertension in the emphysema group increased, and the incidence rate of pulmonary hypertension in the bronchitis group increased. The CAT score of patients with emphysema phenotype of COPD is higher than those with bronchitis phenotype. Besides, lung function decline is more obvious in the former group, showing characteristics of pulmonary hyperventilation and restricted airflow. Objective To investigate the influence of different inspired oxygen concentrations during anesthetic emergence on postoperative atelectasis in elderly patients undergoing elective laparoscopie radical gastrectomy for gastric cancer. Methods Sixty-eight elderly patients scheduled for elective laparoscopic radical gastrectomy were randomly as- signed to either the high inspired oxygen concentration group (Group C) or the low inspired oxygen concentration group (Group L) with 34 patients in each group. During anesthetic emergence, Group L received 40% inspired oxygen, while Group C received 100% inspired oxygen. Bedside lung ultrasound was used to assess lung ultrasound scores (LUS) in 12 regions (6 in each lung) at entry (To) and 30 minutes postoperatively (T1). The occurrence of postoperative atelectasis was recorded. Heart rate (HR), mean arterial pressure (MAP), and blood gas analysis including oxygen partial pressure (PaO2) and oxygenation index (OI) were documented at entry (To), end of pneumoperitoneum (T1), extubation (T2), and PACU discharge (T3). The incidence of SpO2 <94% events in the PACU was recorded. Forced expiratory volume in 1 second (FEV1), forced vital capacity (FVC), and FEV1/FVC were measured at preoperative day 1 (Do) and postoperative days 1 (D1), 3 (D2), and 5 (D3). Clinical Pulmonary Infection Score (CPIS) on postoperative day 1 and the incidence of pulmonary complications on postoperative day 7 were recorded. Results The time from the end of pneumo- peritoneum to extubation was comparable between Group L (36.4±5.2 min) and Group C (37.3±4.2 min) (P> 0.05). Compared with Group C, Group L showed significantly lower LUS and lower incidence of postoperative atelectasis at T4 (P<0.05). At T3, Group L demonstrated significantly increased PaO2 and OI (P<0.05). The incidence of SpO2 <94% events in the PACU was lower in Group L than in Group C (P<0.05). Compared with Do, FEV1, FVC, and FEV1/FVC were significantly reduced in both groups on D1-3 (P<0.05). Compared with Group C, Group L showed significantly increased FEV, FVC, and FEV1/FVC at D1 (P<0.05). There were no significant differences in CPIS on postoperative day 1 and the incidence of pulmonary complications on postoperative day 7 between the two groups (P> 0.05). Conclusion During anesthetic emergence, low inspired oxygen concentration reduces the incidence and severity of postoperative atelectasis in elderly patients undergoing laparoscopic radical gastrectomy for gastric cancer, improving early postoperative lung function. Objective To analyze the relationship between vitamin D levels at birth and the development of bronchopulmonary dysplasia（BPD） and long-term pulmonary function in preterm infants. Methods A total of 120 preterm infants admitted to the neonatal intensive care unit（NICU） between June 2018 and March 2020 were included in this study. Serum vitamin D levels were measured in all preterm infants at birth. Follow-up assessments were conducted when the infants reached 2 years of age, during which pulmonary function parameters, including TPTEF/TE, VPTEF/VE, and PEF, were measured. Spearman correlation analysis was used to assess the relationship between serum vitamin D levels and the development of BPD, while Pearson correlation analysis was used to evaluate the association between serum vitamin D levels at birth and pulmonary function parameters at 2 years of age.Results Compared to preterm infants in the non-BPD group, those in the BPD group had significantly lower serum vitamin D levels（P＜0.05）. Serum vitamin D levels varied significantly among infants with different BPD severity levels（P＜0.05）, with progressively lower levels observed in more severe cases. At 2 years of age, preterm infants in the BPD group exhibited significantly lower pulmonary function parameters, including TPTEF/TE, VPTEF/VE, and PEF, compared to those in the non-BPD group（P＜0.05）. Significant differences in these pulmonary function parameters were also observed among infants with varying BPD severity levels（P＜0.05）, with greater disease severity corresponding to lower pulmonary function parameters. There was a significant negative correlation between serum vitamin D levels at birth and the development of BPD（r=-0.659, P＜0.05）. Moreover, serum vitamin D levels at birth were positively correlated with pulmonary function parameters at 2 years of age, including TPTEF/TE, VPTEF/TE, and PEF（r=0.254, 0.232, 0.265, P＜0.05）. Conclusion Serum vitamin D levels at birth are closely associated with the development of BPD and long-term pulmonary function in preterm infants. Objective: To observe the effects of combined intervention of lumbar core muscle strength training and abdominal breathing training on lung function, lumbar function and lumbar posture control ability of patients with lumbar disc herniation（LDH). Methods: LDH 98 patients who were treated in the First Affiliated Hospital of Hunan University of Traditional Chinese Medicine from October 2020 to October 2022 were selected. Patients were divided into control group（waist core muscle strength training, n=49） and study group（abdominal breathing training on the basis of the control group, n=49） according to the random number table method. The changes of visual analogue scale（VAS), World Health Organization Quality of Life-BREF（WHOQOL-BREF） score, lumbar function, lung function and lumbar posture control ability were compared between two groups. Results: The VAS score in study group was lower than that in control group 3 months after intervention（P＜0.05). The Japanese Orthopaedic Association（JOA） score in study group was higher than that in control group 3 months after intervention. The Oswestry disability index（ODI） score was lower than that in control group（P＜0.05). 3 months after intervention, the maximum ventilation volume（MVV), forced vital capacity（FVC） and maximum expiratory rate in the first second（FEV1） in study group were higher than those in control group（P＜0.05). 3 months after intervention, the root mean square（RMS） of left and right multifidus electromyography and RMS of left and right transverse abdominal muscles in study group were higher than those in control group. The transfer distance of pressure center was less than that in control group（P＜0.05). Objective: To investigate the effects of immune enhanced enteral nutrition on nutritional status, lung function and humoral immunity in elderly patients with severe pneumonia（SP). Methods: 104 elderly SP patients in our hospital from January 2021 to January 2023 were selected as research objects, and were randomly divided into control group and observation group with 52 cases in each group according to random drawing method. The control group received conventional enteral nutrition support, and the observation group received immune enhanced enteral nutrition support. The recovery time of symptoms and signs, severity of disease, pneumonia severity index, lung function, nutritional status, humoral immunity index before and after treatment were compared between the two groups. Results: The disappearance time of fever, cough, lung rales and X-ray shadow in observation group was shorter than that in control group（P＜0.05). After treatment, the Acute Physiology and Chronic Health Evaluation Ⅱ（APACHEⅡ） scores of the observation group were lower than those of the control group（P＜0.05); After treatment, the risk grade of pneumonia in observation group was better than that in control group（P＜0.05). After treatment, the levels of total protein（TP), prealbumin（PA), albumin（ALB） and transferrin（TRF） in observation group were higher than those in control group（P＜0.05); After treatment, the dynamic lung compliance（Cdyn） of observation group was higher than that of control group, and airway resistance（R） was lower than that of control group（P＜0.05). After treatment, the levels of immunoglobulin A（Ig A), immunoglobulin M（Ig M), immunoglobulin G（Ig G), complement C3 and complement C4 in observation group were higher than those in control group（P＜0.05). To explore the efficacy of vitamin D drops combined with budesonide aerosol in the treatment of children with asthma and its effects on lung function, immune function and inflammatory factors. 120 children with asthma admitted to our hospital from August 2019 to December 2022 were randomly divided into observation group and control group, 60 cases in each group. The control group was treated with budesonide aerosol; on the basis of the control group, the observation group was treated with vitamin D drops. The clinical efficacy and clinical symptoms, lung function [forced expiratory volume in the first second (FEV1), peak expiratory flow (PEF), PEF diurnal variation rate], immune function [regulatory T cells (Treg), helper T cells 2 (Th2), Th17] and inflammatory factors [tumor necrosis factor-琢 (TNF-琢), interleukin-6 (IL-6), IL-8] levels before and after treatment were compared between the two groups. The total effective rate of the observation group was higher comparing with the control group (91.67% vs 73.33%, P<0.05). Compared with the control group, the scores of daytime symptoms and nighttime symptoms in the observation group were lower, and the C-ACT score was higher (P<0.05). Compared with the control group, the percentage of FEV1, PEF and Treg in the observation group was higher, and the diurnal variation rate of PEF, the percentage of Th2 and Th17 were lower (P<0.05). After treatment, the percentages of Treg, Th2 and Th17 in the observation group were lower comparing with the control group (P<0.05). The levels of serum TNF-琢, IL-6 and IL-8 in the observation group were significantly lower comparing with the control group (P<0.05). AIM: This study aims to investigate the histopathological and ultrastructural alterations in the lung tissues of rats induced by a single intratracheal administration of bleomycin, with the objective of establishing a reliable model for future applications. METHODS: Six to eight-week-old SD rats were randomly allocated into two groups: the control group and the model group (n=12). Pulmonary fibrosis was induced in the rat models by a single intratracheal instillation of bleomycin (3 mg/kg), while an equivalent volume of saline was administered to the control group. The rats were executed on the 42nd day. Twelve rats remained in the control group, while nine rats remained in the model group. Lung tissue imaging was conducted using CT scans. Lung function tests were performed to assess changes in forced vital capacity (FVC) and dynamic lung compliance (Cdyn). Lung stiffness was determined through Young's modulus testing using a rheometer. The pathological structure of lung tissues was examined using both HE and Masson staining methods. Additionally, transmission electron microscopy was employed to evaluate collagen deposition in lung tissues, alveolar type II epithelial cells, macrophages, and ultrastructural changes of the respiratory membrane. RESULTS: CT scans revealed honeycomb patterns in the lungs of model rats, along with partial bronchiectasis/bronchiectasis. In comparison to the control group, the model group exhibited significantly lower FVC and Cdyn values, while lung stiffness were increased. HE and Masson staining demonstrated that rats in the model group exhibited alveolar structure destruction, alveolar septum thickening, inflammatory cell infiltration, and collagen deposition in alveolar septum. Transmission electron microscopy revealed several abnormalities in the model group: increased collagen fibers in the alveolar septa, misalignment of micro-villi in alveolar type II epithelial cells, wrinkled nuclei with increased heterochromatin, swollen cytoplasmic mitochondria, fractured or haphazardly structured mitochondrion cristaes, and a significant decrease in their number (P<0.05). Furthermore, lamellar bodies were vacuolated and reduced in number (P<0.05), and dilated endoplasmic reticulums with degranulation were observed. There was an increase in alveolar macrophages and interstitial macrophages (P<0.01). The respiratory membrane displayed structural disruptions and an increase in thickness (P<0.01). CONCLUSION: Bleomycin induces decreased lung compliance, alveolar epithelial injury, alveolar septum thickening, collagen deposition, and an increase in interstitial macrophages, ultimately resulting in pulmonary fibrosis in rats. Objective To investigate the influencing factors of abnormal pulmonary function in workers exposed to dust in Yinchuan, and to establish and verify a risk prediction model of abnormal pulmonary function. Methods A total of 923 dust workers who underwent occupational health examination in Yinchuan from March to July 2022 were investigated by questionnaire, physical examination, and laboratory examination. The influencing factors of abnormal pulmonary function in dust exposed workers were analyzed by Logistic regression, and the risk prediction model of abnormal pulmonary function was established> The Receiver Operator Characteristic (ROC) curve, calibration curve, and decision analysis curve were used to evaluate the model. Results The results of multivariate Logistic regression showed that no personal protection (OR =7.806, 95%CI: 5,803-11.989), length of service at work (compared with 0 to 5 years, 6 to 10 years: OR =3.025, 95%CI: 1.764-5.18& > 10 years: OR =2.546: 95%CI: L545-4.193), hypertension (OR =3.909: 95%CI: 2.223-6.873), and BMI (3 24.0 kg/m2) (OR=1.553, 95%CI: 1.004-2.401) were independent risk factors of pulmonary dysfunction, while education (college) (OR= 0,503, 95%CI: 0.299-0.847) was the protective factor of pulmonary dysfunction. The area under the ROC curve of the risk prediction line chart was 0.806, and the calibration curve showed good consistency. The results of clinical decision curve showed that the intervention of abnormal lung function had greater benefits when the threshold probability of the line chart model was 10% to 88%. Conclusion The nomogram constructed by Logistic regression visually demonstrates the role of many influencing factors and has good accuracy in predicting the risk of abnormal pulmonary function in workers exposed to dust. Objective: To investigate the effects of Yifei capsule combined with umeclidinium bromide and vilanterol powder for inhalation on lung function, cellular immune function, serum basic fibroblast growth factor (bFGF) and silent information regulator 1 (SIRTI) in patients with chronic obstructive pulmonary disease (COPD) at stable stage. Methods: 104 patients with COPD at stable stage who were admitted to our hospital from March 2021 to June 2022 were select and they were divided into control group (treatment with umeclidinium bromide and vilanterol powder for inhalation, n=52) and observation group (treatment with Yifei capsule combined with umeclidinium bromide and vilanterol powder for inhalation, n=52) according to random number table method. Clinical symptom recovery time, 6min walking test (6MWT) and COPD assessment test (CAT) score, lung function [forced expiratory volume in the first second (FEV1), forced vital capacity (FVC), maximum gas volume per minute (MVV)], cellular immune function and serum bFGF and SIRTI levels in the two groups were compared. The occurrence rate of adverse reactions in the two groups was observed. Results: Compared with the control group, the disappearance time of lung rales, cough and sputum in the observation group were shortened (P<0. 05). Compared with the control group, the 6MWT in the observation group at 14 d after treatment was longer, and the CAT score was lower (P<0.05). AIM: To investigate the effect of recombinant human CC16 protein （rhCC16） on lung function of chronic obstructive pulmonary disease （COPD） mice, and to reveal its underlying mechanism. METHODS: Forty male BALB/c mice were randomly divided into control group, COPD group, COPD+rhCC16 group and COPD+PBS group, with 10 mice in each group. The COPD mice were induced using passive smoking for 6 months. From the beginning of 16 weeks of smoking, the mice were intranasally administered with rhCC16（ 2. 5 μg/g） or equal volume（ 20 μL） of PBS at 2 h before smoking in COPD+rhCC16 group or COPD+PBS group. The lung function indexes were examined and the morphological changes of lung tissues were observed by HE staining. The mRNA levels of P16 and P21 in lung tissues were tested by RT-qPCR, and the protein levels of P16, P21, senescence-associated β-galactosidase （SA-β-Gal）, p-P38 and phosphorylated extracellular signal-regulated kinase 1/2 （p-ERK1/2） were detected by immunohistochemical staining. RESULTS: Compared with control group, the forced expiratory volume in 0. 3 s （FEV0. 3）/forced vital capacity （FVC） ratio and the lung compliance were declined in COPD group （P<0. 05）, the airway resistance was enhanced （P<0. 05）. Compared with COPD group, the FEV0. 3/FVC ratio and lung compliance were elevated in COPD+rhCC16 group （P<0. 05）, and the airway resistance was reduced （P<0. 05）. Compared with the control mice, the alveolar walls were ruptured and collapsed in COPD mice, and there were uneven alveolar size, irregular enlargement, luminal narrowing and thickened walls. Nevertheless, rhCC16 treatment mitigated the pathological damages of lung tissues compared with COPD mice. Compared with control group, the mRNA levels of P16 and P21 in lung tissues were significantly increased in COPD group （P<0. 05）, and the protein levels of P16, P21, SA-β-Gal, p-P38 and p-ERK1/2 in lung tissues of COPD mice were also significantly augmented （P<0. 05）. Compared with COPD group, the mRNA levels of P16 and P21 in lung tissues were significantly decreased in COPD group （P<0. 05）, and the protein levels of P16, P21, SA-β-Gal, p-P38 and p-ERK1/2 in lung tissues were also significantly inhibited （P<0. 05）. The lung function indexes, pathological changes, mRNA or protein of P16, P21, SA-β-Gal, p-P38 and p-ERK1/2 showed no significant difference between PBS group and COPD group. Objectives: This study investigated the effectiveness of photocatalytic oxidation (PCO) and air filtration technology for removing indoor air pollutants and improving the health of patients with asthma. Methods: A double-blind crossover study was conducted. In total, 58 patients with asthma residing in a heavily industrialized area were recruited. The patients were randomly assigned to two groups: a PCO group and a PCO+filter group. The PCO+filter group underwent an intervention that consisted of a 2-week period using a PCO and a filter, a 2-week washout period, and a 2-week period using only a PCO. The PCO group followed the same sequence, with interventions reversed. During the intervention period, measurements were taken to assess the removal of indoor particulate matter (PM1, PM2.5, PM4, PM10, TSP, and UFPs), gaseous pollutants (CO, SO2, NO2, CO2, and TVOC), and their effect on health outcomes, including lung function, blood pressure, FeNO levels, ACT score, and symptoms. Results: Significant reductions in indoor gaseous pollutants, such as SO2 and TVOC, were observed in the PCO group. Significant reductions in PM, CO, NO2, and TVOC were observed in the PCO+filter group. Improvements in lung function were observed after only 7 days, and significant reductions in blood pressure were observed after 13 days. Both groups reported improvements in symptoms. Objective To explore the application value of BiPAP non-invasive ventilator in patients with chronic obstructive pulmonary disease (COPD) combined with chronic respiratory failure. Methods The data of 100 COPD patients combined with chronic respiratory failure who were cured in the First Affiliated Hospital of Chongqing Medical University and Chongqing General Hospital from January 2021 to January 2022 were collected and analyzed. A total of 100 patients were divided into the control group and the non-invasive positive pressure ventilation (NIPPV) group, with 50 patients in each group. The control group was given routine oxy- gen therapy, while the NIPPV group was given BiPAP non-invasive ventilator on the basis of routine oxygen therapy. The blood gas analysis, pulmonary function, six-min walk test (6MWT).readmission times and hospitalization expenses were evaluated before and after treatment for 6 months. Results Compared with the control group, the arterial partial pressure of carbon dioxide (PaCO2) and arterial partial pressure of oxygen (PaO2) in NIPPV group were significantly improved (P<0.05). However, the improvement of arterial oxygen saturation (SaO2) and pH was not obvious, and the differences were not statistically significant (P>0.05). Compared with the control group, the measured/predicted forced expiratory volume in the first second (FEV1), FEV1/forced expiratory volume (FVC) and 6MWT in NIPPV group were improved more obviously. and the differences were statistically significant (P<0.05). The number of readmissions and hospitalization expenses in NIPPV group were better than those in the control group, and the differences were statistically significant (P<0.05). Conclusion BiPAP ventilator can improve hypercapnia and hypoxemia in COPD patients combined with chronic respiratory failure, improve their lung function and quality of life, avoid the deterioration of their condition, reduce the number of re-hospitalizations, and significantly reduce the economic burden of families and society. Objective: To investigate the relationship between serum micro ribonucleic acid (miR)-29a-3p and miR-98-5p expression levels and lung function, airway inflammation and sensitivity to glucocorticoid (GC) therapy in patients with bronchial asthma (BA). Methods: 150 patients with BA who were admitted to Weifang People's Hospital from January 2020 to January 2022 were selected as BA group. According to sensitivity to GC therapy of patients with BA, they were divided into resistance group with 43 cases and sensitive group with 107 cases, and 57 healthy subjects during the same period were selected as control group. The data of lung function and airway inflammation indexes in the BA group and control group were collected, and the serum miR-29a-3p and miR-98-5p expression levels in the two groups were detected by real-time fluorescence quantitative reverse transcription polymerase chain reaction (qRT-PCR). Spearman correlation was used to analyze the correlation between the serum miR-29a-3p and miR-98-5p expression levels in patients with BA and lung function and airway inflammation indexes, and univariate and multivariate Logistic regression were used to analyze the influencing factors of GC resistance in patients with BA. Results: Compared with the control group, the serum miR-29a-3p and miR-98-5p expression levels, the percentage of forced expiratory volume in the first second to the predicted value (FEV1%), forced expiratory volume in one second/forced vital capacity (FEV1/FVC) and peak expiratory flow (PEF) in the BA group were decreased, the fractional exhaled nitric oxide (FeNO) level was increased (all P<0.05). Spearman correlation analysis showed that the serum miR-29a-3p and miR-98-5p expression levels in patients with BA were positively correlated with FEV1%, FEV1/FVC and PEF (all P<0.05), and negatively correlated with FeNO level (all P<0.05). Univariate analysis showed that the body mass index>24 kg/m², smoking ratio in the resistance group were higher than those in the sensitive group, and the serum miR-29a-3p and miR-98-5p expression levels were lower than those in the sensitive group (P<0.05). Multivariate Logistic regression analysis showed that body mass index>24 kg/m², smoking were independent risk factors for GC resistance in patients with BA, and increased serum miR-29a-3p and miR-98-5p expression levels were independent protective factors (all P<0.05). Conclusion: Serum miR-29a-3p and miR-98-5p levels decreased in patients with BA, which are associated with decreased lung function, airway inflammation and resistance to GC therapy. Objective: To observe the effect of heated humidified high-flow nasal cannula (HFNC) on blood gas index, lung function and cytokine level in children with severe pneumonia with respiratory failure. Methods: 86 children with severe pneumonia with respiratory failure who were admitted to Children’s Hospital Affiliated to Nanjing Medical University from March 2020 to March 2022 were selected, and they were divided into nasal continuous positive airway pressure (nCPAP) group and HFNC group according to random number table method, with 43 cases in each group. The clinical related indexes, blood gas indexes, lung function and cytokine levels were compared in the two groups, and the sedative use, prognosis and complications were observed in the two groups. Results: The mechanical ventilation time, cough relief time, lung rales disappearance time and hospitalization time in children’s intensive care unit (PICU) in the HFNC group were shorter than those in the nCPAP group (P＜0. 05). Heart rate (HR) increased, and respiratory rate (RR)decreased in the two groups after treatment, and the changes in the HFNC group were greater than those in the nCPAP group (P＜0. 05). After treatment, pH value, blood oxygen partial pressure (PO2), blood oxygen saturation (SpO2) and oxygenation index (OI) in the two groups increased, and HFNC group was higher than nCPAP group (P＜0. 05). Objective To investigate the adverse effects of low-concentration isocyanate exposure on airway inflammation and lung function and to explore the application of fractional exhaled nitric oxide (FeNO) in occupational health examinations of isocyanate-exposed workers. Methods From 2016 to 2017, a total of 192 workers exposed to isocyanates were selected as the exposure group, and 91 with similar age and gender composition (office and/or logistics personnel) without any exposure to occupational hazards (as references) were studied. Monitoring data of airborne isocyanate concentration in workplaces in the past 5 years were collected. The basic information of all workers was collected, and their lung function, FeNO, and salbutamol bronchial dilation tests were performed. The differences in various indicators among these two groups of workers were compared. Results The airborne isocyanate concentration in workplaces over the past 5 years was significantly lower than the national occupational exposure limit. The forced vital capacity (FVC) and forced expiratory volume in first second (FEV1) of the exposed workers were higher than those of the reference group, while FEV1/FVC was lower than that of the reference group, and the differences were statistically significant (P < 0.001). There was no significant difference in FeNO levels between the exposed workers and the reference workers (P > 0.05). The FEV1/FVC ratio was higher in the exposed workers with exposure time < 7 years than workers with exposure time≥ 7 years (P < 0.05), while there was no significant difference in FeNO levels between these two groups (P > 0.05). There was no significant difference in various indicators of lung function among different workers grouped according to the FeNO levels (P > 0.05). Among 192 exposed workers, 13 had FeNO levels higher than 50 ppb (1 ppb = 1 × 10-9), and among 9 workers receiving bronchial dilation tests 3 workers were positive. Conclusions FEV1/FVC could better reflect the adverse effect of isocyanate, a type of asthmagens, on lung function. FeNO levels cannot indicate the occupational low -level isocyanate exposure, while positive changes shown by bronchial dilation tests in exposed populations occur earlier than changes in lung function. Objective: To investigate the correlation between serum progranulin (PGRN), secreted frizzled related protein 1 (SFRP1) and C-C motif chemokine ligand 26 (CCL26) and lung function and airway inflammation in patients with acute attack stage of bronchial asthma (BA). Methods: 118 patients with acute attack stage of BA who were admitted to our hospital from January 2021 to June 2022 were selected as the acute attack stage group. According to the disease grade, the patients with acute attack stage of BA were divided into mild subgroup with 55 cases, moderate subgroup with 43 cases and severe subgroup with 20 cases. Another 77 patients with clinical control period of BA (clinical control period group) and 60 healthy volunteers in physical examination (control group) were selected as controls respectively in the same period. Pearson correlation was used to analyze the correlation between serum PGRN, SFRP1, CCL26 levels and lung function and airway inflammation indicators in patients with acute attack stage of BA. Results: The serum PGRN level, the forced expiratory volume in one second % (FEV1%pred) and the peak expiratory flow (PEF) in the control group, the clinical control period group and the acute attack stage group were decreased successively, the SFRP1 and CCL26 levels, fractional exhaled nitric oxide (FeNO) and peripheral blood eosinophil (EOS) count were increased successively (P<0.05). Conclusion: Patients with acute attack stage of BA have reduced serum PGRN level and increased SFRP1 and CCL26 levels, which are associated with disease severity, lung function and airway inflammation, and which may become a new therapeutic target for patients with acute attack stage of BA. Objective To investigate the correlation between pulmonary function and respiratory muscle strength in adolescent idiopathic scoliosis (AIS） patients compared to healthy subjects of the same age, and to investigate the correlation between the size of the scoliosis angle and the type of scoliosis with pulmonary function and respiratory muscle strength in AIS patients. Methods We analyzed the lung function indicators of 50 patients with AIS ( Cobb=29.720±11.875） and 50 healthy adolescents treated at the Department of Orthopedics, Second Affiliated Hospital of Kunming Medical University from June to December 2022. Pulmonary function indicators included forced vital capacity ( FVC） as a percentage of predicted value ( FVC pred%), and forced expiratory volume in one second ( FEV1). ( FEV1 pred%), maximum expiratory flow volume ( PEF pred%), and the maximum expiratory rate in one second (FEV1/FVC). Respiratory muscle function indicators included maximum expiratory pressure ( MEP） and maximum inspiratory pressure ( MIP). Results There were no statistical differences in FEV1/FVC and PEF pred% between the AIS group and healthy controls, and FVC pred%, FEV1 pred% (P < 0.01), inspiratory muscle strength, and expiratory muscle strength were worse in the AIS group than in the healthy group. There was no correlation between pulmonary function parameters, respiratory muscle strength, and Cobb’s angle, and no difference in pulmonary function and respiratory muscle strength between patients with different scoliosis types. Conclusions Patients with AIS have poorer FVC pred% and FEV1 pred%, and respiratory muscle strength than healthy subjects of the same age. Objective: To investigate the effects of tuberculosis pills combined with cycloserine containing chemotherapy on lung function and peripheral blood T helper cells (Th)1/Th17 cells immune responses of peripheral blood in patients with multidrug-resistant tuberculosis (MDR-TB). Methods: 98 MDR-TB patients who were admitted to our hospital from May 2018 to March 2020 were divided into control group (49 cases, cycloserine-containing chemotherapy regimen) and observation group (49 cases, tuberculosis pill combined with cycloserine-containing chemotherapy regimen) according to envelope lottery method. Sputum negative conversion rate, lesion absorption rate, cavity closure rate, lung function index, peripheral blood Th1/Th17 cell immune response related index and incidence of adverse reactions were compared in the two groups. Results: The lesion absorption rate, sputum negative conversion rate and cavity closure rate in the observation group were higher than those in the control group, and the total incidence of adverse reactions was significantly lower than that in the control group(P<0.05). After treatment, the maximum expiratory flow (PEF), forced expiratory volume in the first second (FEV1) and forced vital capacity (FVC) in the observation group were higher than those in the control group at the same period (P<0.05). After treatment, Th1 and γ-interferon (IFN-γ) in the observation group were higher than those in the control group at the same period, and Th17 and interleukin (IL)-17 were lower than those in the control group at the same period (P<0.05). To investigate the main factors affecting the pulmonary function of the dust -exposed workers in Guiyang City. Methods A total of 964 workers who had undergone the occupational health examination during the on - the -job period were selected from Guiyang Public Health Treatment Center in the period of January 2020 to December 2020 and divided into the dust-exposed group (n = 526) and the control group (n = 438). The basic characteristics and lung function examination results of workers in these two groups were analyzed. Multiple logistic regression was used to analyze the factors affecting the lung function of the dust-exposed workers. Results The incidence of abnormal high -kV chest radiograph (34.60%), chest tightness (19.39%), shortness of breath (10.27%), cough (23.76%), and sputum (18.06%) were all significantly higher in the dust -exposed group than those in the control group (21.69%, 1.83%, 1.14%, 13.70%, and 7.08%, respectively), and the differences were all statistically significant (P < 0.001). The lung function indexes, including the FVC, FEV1, and FEV1/FVC, of workers in the dust-exposed group were significantly lower than those of workers in the control group (P < 0.001). The incidence of abnormal lung function in workers in the dustexposed group (26.62%) was significantly higher than that of workers in the control group (13.24%) (P < 0.001). Multivariate logistic regression analysis showed that working time and age growth were the risk factors for abnormal lung function in dust-exposed workers (OR = 1.292-1.560, P < 0.05). Compared with those exposed to welding dust, the risk of abnormal lung function in workers exposed to silica dust and coal dust increased by 1.510 and 1.428 times, respectively (P < 0.05). Frequent wearing of protective equipment was a protective factor for abnormal lung function (OR = 0.594). Compared with the dust-exposed workers in large enterprises, the risk of abnormal lung function in dust-exposed workers in small and medium-sized enterprises was increased (OR = 1.259, 1.042, P < 0.05). Objective: To compare the influence of emphysema and lung small shadow on the lung function of patients with pneumoconiosis. Methods: We selected 96 patients who were admitted to the Taihe Hospital (Shiyan city) for pneumoconiosis complicated by emphysema between 2015 and 2020. Chest computed tomography (CT) and lung function assessments were performed. The low attenuation volumes (LAV%) of the whole lung; left and right lungs; and upper, middle, and lower lobes of each lung were quantified using 64-slice spiral CT (GE Optima CT680). Based on the LAV% of the whole lung, the patients were divided into groups A (LAV%30%). The small shadow density (SSD) of the whole lung; left and right lungs; and upper, middle, and lower lung fields was calculated by applying the criteria for evaluating SSD on chest radiographs to CT images. The correlation between SSD and lung function was also analyzed. Results: The SSD of the whole lung was negatively correlated with the forced expiratory volume in the first second (FEV1), total lung capacity (TLC), peak expiratory flow (PEF), maximum expiratory flow after 75% of the forced vital capacity (FVC) has not been exhaled (MEF75%), and maximum expiratory flow after 25% of the FVC has not been exhaled (MEF25%), and it was positively correlated with residual volume (RV)/TLC. LAV% was negatively correlated with FEV1, ratio of FEV to FEV1 (FEV1%), carbon monoxide diffusion capacity (DLCO), PEF, MEF75%, maximum expiratory flow after 50％ of the FVC has not been exhaled (MEF50%), and MEF25% in the whole lung; left and right lungs; and upper, middle, and lower lung lobes; however, it was positively correlated with RV and RV/TLC%. When LAV% was >30%, the correlation between LAV% and FEV1, FEV1%, DLCO, RV/TLC, PEF, MEF75%, MEF50%, and MEF25% was stronger in group C than in groups A and B. A certain degree of correlation was observed between the SSD in the lungs and the ventilation function and volume parameters; however, the correlation between SSD and the diffusion function was weak. Conclusion: The degree of emphysema and SSD in patients with pneumoconiosis have a certain degree of correlation with the lung function; however, the SSD of emphysema has a relatively better correlation with the lung function. It can be used to evaluate the degree of lung injury in patients with pneumoconiosis to a certain extent and is worthy of clinical application.

Objective: To investigate the correlation between serum complement C1q/tumor necrosis factor-related protein 4 (CTRP-4), CTRP-6 and CTRP-9 with lung function and inflammatory factors in elderly patients with acute exacerbation of chronic obstructive pulmonary disease (AECOPD). Methods: 117 patients with AECOPD who were admitted to our hospital from January 2020 to January 2022 were selected as the AECOPD group, 72 patients with stable chronic obstructive pulmonary disease (SCOPD) were selected as the SCOPD group, and another 58 healthy people with physical examination during the same period were selected as the control group. Serum CTRP-4, CTRP-6 and CTRP-9 levels were measured by enzyme-linked immunosorbent assay, and the Pearson/Spearman correlation were used to analyze the correlation between serum CTRP-4, CTRP-6 and CTRP-9 and lung function indexes [forced expiratory volume in one second as a percentage of projected (FEV1%), forced expiratory volume in one second/forced vital capacity (FEV1/FVC)] and inflammatory factors [interleukin (IL)-1β, IL-6, IL-8, tumor necrosis factor-α (TNF-α)] in elderly patients with AECOPD. Results: The serum CTRP-4 level in the control group, SCOPD group and AECOPD group increased in turn, while the CTRP-6and CTRP-9 levels decreased in turn (P＜0.05). Serum CTRP-4 level in elderly patients with AECOPD were negatively correlated with FEV1% and FEV1/FVC, and positively correlated with serum IL-1β, IL-6, IL-8, and TNF-α levels (P＜0.05). CTRP-6 and CTRP-9 levels were positively correlated with FEV1% and FEV1/FVC, and negatively correlated with serum IL-1β, IL-6, IL-8 and TNF-α levels (P＜0.05). Conclusion: The increased serum CTRP-4 level and the decreased CTRP-6 and CTRP-9 levels are associated with the decreased lung function and inflammatory response in elderly patients with AECOPD, which may be a potential target for the prevention and treatment of AECOPD in the elderly. Objective To explore the correlation between pulmonary function and chest radiograph fibrosis score among patients with pneumoconiosis. Methods A total of 100 patients with pneumoconiosis treated at the Shandong Academy of Occupational Health and Occupational Medicine from November 2020 to April 2021 were studied. The chest digital radiography of these patients was evaluated, and the fibrosis score was assessed based on the density of shadow in each lung area on the chest X -ray. The relationship between the score of pulmonary fibrosis and pulmonary function was analyzed. Results Among 100 male cases of pneumoconiosis, 37 cases were in the first stage, 41 cases in the second stage, and 22 cases in the third stage. 34 cases had lung function impairment, including 22 mild, 11 moderate, and 1 severe case, with an incidence of 27.02% in the first stage, 29.26% in the second stage, and 54.54% in the third stage; the difference was significant (P < 0.05). The fibrosis score increased with the pneumoconiosis stage (P < 0.01), and the forced vital capacity (FVC), forced expiratory volume in one second (FEV1), FEV1/FVC, 75% maximal expiratory flow (MEF75), and 50% maximal expiratory flow (MEF50) decreased (P < 0.05). The scores of pulmonary fibrosis were negatively correlated with FVC, FEV1, MEF75, MEF50, 25% vital capacity (MEF25), and the single breath diffusing capacity for carbon monoxide (DLCO SB) (r = - 0.339, - 0.431, - 0.334, - 0.362, - 0.369, - 0.245, P < 0.05). The patients with more severe pulmonary function had a higher score of fibrosis (P < 0.05). Conclusions The fibrosis score on the chest radiograph of pneumoconiosis patients is a useful indicator for the clinician to evaluate the lung function damage caused by pneumoconiosis. Objective To compare the effects and characteristics of cigarette smoke exposure (CSE) alone and CSE combined with airway instillation of bacterial lipopolysaccharide (LPS) in chronic obstructive pulmonary disease (COPD).Methods Male SD rats were randomly divided into control, CSE, and CSE+LPS groups, with 10 rats in each group. After 24 weeks, the models were established and the lung function of the rats was measured. Hematoxylin-eosin (HE) staining was performed to observe the pathological changes in the airway and lung tissue. The ELISA method was used to detect the level of serum interleukin-8 (IL-8) and tumor necrosis factor α (TNF-α) in peripheral blood.Results Airway resistance (RI), functional residual capacity (FRC), and chord compliance (Cchord) of the CSE and LPS+CSE groups were higher than those of the control group (all P < 0.05), while the tidal volume (TV), minute volume (MV), and forced expiratory volume in 50 ms (FEV50) / forced vital capacity (FVC) of the CSE and LPS+CSE groups were lower than those of the control group (all P < 0.05). HE staining of lung tissue showed that the average alveolar intercept and thickness of the small airway walls were higher in the CSE and LPS+CSE groups than those in the control group. Compensatory enlargement was evident in the alveolar cavity of the CSE and CSE+LPS groups, and the alveolar septum widened, with a fusion of pulmonary alveoli in the CSE+LPS group. The levels of IL-8 and TNF-α in serum of the CSE and CSE+LPS groups were higher than those of the control group (all P < 0.05). The level of TNF-α in serum of the CSE+LPS group was higher than that of the CSE group (P < 0.05).Conclusion The CSE combined with LPS method is superior to CSE alone for establishing the COPD rat model, and the combined model is closer to clinical manifestations.

Objective: To investigate the effects of Feilike Heji combined with budesonide and compound ipratropium bromide on lung function, inflammatory mediators and immune function in children with bronchial asthma. Methods: 76 children with bronchial asthma who were received by the pediatric department of our hospital from February 2021 to March 2022 were selected as the observation objects. The children were divided into the control group (n=38, budesonide and compound ipratropium bromide treatment) and the study group (n=38, Feilike Heji combined with budesonide and compound ipratropium bromide treatment) by envelope lottery. The curative effect, lung function, inflammatory mediators and immune function of the two groups were compared, at the same time, the drug safety status of the two groups during treatment was recorded. Results: The total clinical effective rate in the study group was significantly higher than that in the control group (P<0.05). 2 weeks after treatment, forced expiratory volume in the first second (FEV1), forced expiratory flow (PEF) and forced vital capacity (FVC) in the study group were higher than those in the control group (P<0.05). 2 weeks after treatment, interleukin-6 (IL-6), tumor necrosis factor-α (TNF-α) and C-reactive protein (CRP) in the study group were lower than those in the control group, and interferon-γ (IFN-γ) was higher than that in the control group (P<0.05). 2 weeks after treatment,CD8+in the study group was lower than that in the control group, and CD3+, CD4+, CD4+/CD8+ were higher than those in the control group (P<0.05). There was no significant difference in the incidence of adverse reactions between groups (P>0.05). Conclusion: Feilike Heji combined with budesonide and compound ipratropium bromide in the treatment of children with bronchial asthma can effectively improve the lung function and immune function of children, reduce the level of inflammatory mediators, and which has a reliable curative effect. Background Studies on tuberculosis associated obstructive pulmonary disease (TOPD) are still scarce in China, a country with a high burden of tuberculosis and chronic obstructive pulmonary disease (COPD) . Objective To compare clinical features and pulmonary functions between TOPD and COPD, and to explore the relationship between imaging features and pulmonary function in TOPD, aiming at providing information for improving clinicians' understanding of TOPD. Methods A retrospective design was adopted. TOPD (n=387) and COPD (n=364) patients were recruited from the Fourth Clinical Medical College of Xinjiang Medical University from January 2018 to November 2020. The TOPD patients were matched with COPD patients at a ratio of 1∶2 using propensity score matching based on age, gender, ethnic group and other baseline data. Clinical data and lung function indices (FEV1/FVC, FEV1%pre, and FVC%pre) were compared between the groups before and after matching. Imaging manifestations of TOPD patients were further described. Spearman rank correlation analysis was used to analyze the correlation of imaging features with lung function. BackgroundThere are daytime variability in pulmonary function indexes such as peak expiratory flow (PEF) and forced expiratory volume in 1 second (FEV1) in asthma patients. Studies evaluating the effects of drug therapy on lung function and airway hyperresponsiveness (AHR) in asthma patients all required patients to perform spirometry and bronchial challenge test in the same time point of the days. However, whether there is a daily diurnal AHR variability is still not clear.ObjectiveTo explore the characteristics and diurnal variability of AHR in asthma patients.MethodsThe data of 202 patients with asthma who consulted in respiratory department of the First Affiliated Hospital of Guangzhou Medical University from January 2018 to September 2020 were included for statistical analysis. All patients completed the methacholine bronchial provocation tests, they were divided into the morning detection group (morning group) with 81 cases and the afternoon detection group (afternoon group) with 121 cases; according to the disease course, 98 cases were divided into the initial diagnosis group if the disease course was ≤6 months, and 104 cases were divided into the follow-up group if the disease course was >6 months. The initial diagnosis group and the follow-up group were divided into the initial diagnosis morning group, the initial diagnosis afternoon group, the follow-up morning group, and the follow-up afternoon group according to the detection time; according to the AHR, the patients were divided into very mild, mild, moderate and severe groups. BackgroundThe degree of airflow limitation is a key indicator of the progression degree in COPD patients. However, problems such as contraindications to testing and compliance make it difficult for some patients to undergo the relevant tests and evaluate the severity of the disease.ObjectiveTo develop and evaluate a machine learning algorithm-based early warning model for the risk of severe airflow limitation in COPD patients.MethodsA cross-sectional design was used to investigate COPD inpatients in a tertiary hospital in Sichuan Province from 2019-01 to 2020-06. General clinical indexes and pulmonary function test data were collected. The data were randomly divided into training and test sets in the ratio of 8∶2, and 216 risk warning models were constructed in the training set using four missing value filling methods, three feature screening methods, 17 machine learning and one integrated learning algorithm. The area under the ROC curve (AUC) , accuracy, precision, recall and F1 score were used to evaluate the predictive performance of the model; and the ten-fold cross-validation method and Bootstrapping were used for internal and external validation, respectively. The test set data was used for model testing and selection, the posterior method was used for sample size verification.ResultsA total of 418 patients were included, of which 212 (50.7%) patients were at risk of severe airflow limitation. After four missing value treatments and three feature filters, a total of 12 processed datasets and the importance ranking of 12 factors affecting airflow limitation were obtained, and the results showed that modified medical research council dyspnea scale grade (mMRC) , age, body mass index (BMI) , smoking history (yes, no) , chronic obstructive pulmonary disease assessment test (CAT) score, and dyspnea (yes, no) were at the forefront inthe ranking of variable features and were key indicators for constructing the model, which had an important role in predicting the outcome. Using unfilled, Lasso screening, mMRC grade, smoking history (yes, no) , and dyspnea (yes, no) were the top 3 predictors, with mMRC grade accounting for 54.15% of feature importance. In which, using unfilled, Boruta screening, CAT score, age, and mMRC class were the top 3 predictors, and CAT score accounted for 26.64% of feature importance. Lung cancer patients complicated with COPD have decreased cardiopulmonary function and increased perioperative risk, and their postoperative exercise endurance and lung function are significantly lower than those with conventional lung cancer. Previous studies have shown that postoperative exercise training can improve the exercise endurance of unselected lung cancer patients, but it is unclear whether lung cancer patients with COPD can also benefit from postoperative exercise training. This study intends to explore the effects of postoperative exercise training on exercise endurance, daily activity and lung function of lung cancer patients with COPD. Methods Seventy-four patients with non-small cell lung cancer (NSCLC) complicated with COPD who underwent pneumonectomy in the lung cancer center of West China Hospital of Sichuan University from August 5, 2020 to August 25, 2021 were prospectively analyzed. They were randomly divided into exercise group and control group; The patients in the two groups received routine postoperative rehabilitation in the first week after operation, and the control group was given routine nursing from the second week. On this basis, the exercise group received postoperative exercise rehabilitation training for two weeks. Baseline evaluation was performed 3 days before operation and endpoint evaluation was performed 3 weeks after operation. Results The exercise endurance, daily activity and pulmonary function test results of the two groups decreased from baseline to the end point. However, after the operation and intervention program, the maximum oxygen consumption of Cardiopulmonary Exercise Test and the walking distance of 6-Minute Walking Test in the exercise group were significantly better than those in the control group [(13.09±1.46) mL/kg/min vs (11.89±1.38) mL/kg/min, P=0.033; (297±46) m vs (243±43) m, P=0.041]. The average number of we-chat steps in the exercise group was also significantly better than that in the control group (4,381±397 vs 3,478±342, P=0.035). Forced vital capacity (FVC) and forced expiratory volume in one second (FEV1) in the exercise group were better than those in the control group, but the difference did not reach a statistically significant level [(1.76±0.19) L vs (1.60±0.28) L, P=0.084; (1.01±0.17) L vs (0.96±0.21) L, P=0.467]. Conclusion Postoperative exercise rehabilitation training can improve exercise endurance and daily activity ability of patients with lung cancer complicated with COPD and promote postoperative rehabilitation.

Objective: To investigate the expression of serum mast cell carboxypeptidase (mc-cp) and Th17 in elderly patients with bronchial asthma and their correlation with respiratory function and pulmonary function. Methods: 120 elderly patients with bronchial asthma who were treated in our hospital from April 2018 to August 2021 were selected as the study group (SG), and 100 patients with bronchitis who were treated in our hospital during the same period were selected as the matched group (CG). The differences in serum mc-cp, Th-17, peak expiratory flow rate (PEF), partial pressure of oxygen (PaO2), FEV1 and FEV1/FVC were compared between the two groups, Spearman analysis was carried out on the correlation between the levels of mc-cp and Th-17 and their respiratory and pulmonary functions in the study group. Results: (1) the level of mc-cp in the study group was higher than that in the matched group, and the percentage of Th17 in the study group was also higher than that in the matched group (P<0.05); (2) PEF, PaO2, FEV1 and FEV1/FVC in the study group were lower than those in the matched group (P<0.05); (3) Mc-cp was negatively correlated with PEF and PaO2 in the study group (r=-0.558, -0.700, P<0.001), and Th17 was negatively correlated with PEF and PaO2 in the study group (r=-0.695, -0.774, P<0.001); (4) Mc-cp was negatively correlated with FEV1 and FEV1/FVC in the study group (r=-0.609, -0.481, P<0. 001), and Th17 was negatively correlated with FEV1 and FEV1/FVC in the study group (r=-0.622, -0.561, P<0.001). Conclusion: The levels of serum mc-cp and Th17 in elderly patients with bronchial asthma will increase abnormally, while the respiratory function and pulmonary function will decrease. Objective To study the application value of graded rescue airway management model for smoke inhalation injury patients. Methods A total of 40 patients with smoke inhalation injury in the hospital from August 2016 to August 2018 were selected as the control group, and 40 patients with smoke inhalation injury from September 2018 to August 2020 were selected as the observation group. The patients in the control group were given the routine airway management, while the patients in the observation group were given a graded rescue airway management model. The score of lung function, psychological state and quality of life were compared. Results After the intervention, forced expiratory volume in 1 second (FEV1), forced vital capacity (FVC), FEV1/FVC, and FEV1 as a percentage of predicted value (FEV1% pred) in both groups were significantly higher than those before the intervention, and the above indexes in the observation group after the intervention were significantly higher than those in the control group at the same time, with statistical significance (P <0. 05). The score of Self-rating anxiety scale (SAS), self-rating depression scale (SDS) of the two groups after the intervention were significantly lower than those before the intervention, and the SAS and SDS scores of the observation group after the intervention were significantly lower than those of the control group at the same period, with statistical significance (P <0. 05). The scores of overall health, mental health, physiological function, physiological function, emotional function, social function, somatic pain and energy of the patients in the two groups after the intervention were significantly higher than those before the intervention.
