An 80-year-old man experienced lack of efficacy during treatment with amiodarone, calcium gluconate, magnesium sulfate, and sodium bicarbonate for monomorphic ventricular tachycardia. Prior to seeking medical help, he had consumed Nirmashi (delphinium species) to treat gastric acidity, resulting in poisoning symptoms. Despite receiving various treatments, including lidocaine and phenytoin, he continued to have cardiac issues, eventually requiring vasopressor support before gradually recovering and being discharged after six days of hospitalization.
