Aims: to prepare Indomethacin mouth wash and study some of its properties (IR, pH, Density, Viscosity, Surface tension). Also, the effect on gingival index score were evaluated. Indomethacin mouth wash was prepared in a concentration of (0.05%) distil water from pure Indomethacin powder. Methods: The study was carried out on 40 subjects aged (20-30) years old. They were divided into four groups ten for each, three groups complaining of gingivitis. The gingival index score for all were measured before and after treatment, treatment involved mechanical treatment by scaling and polishing at dental clinic and drug treatment in which the first group received Indomethacin mouth wash, second group received Chlorhexidine mouth wash and third group received distal water as a mouth wash three times daily for three days, while the fourth group consisted of 10 volunteer subjects without gingivitis. Data were analyzed using paired t-test, ANOVA test and Duncan's Multiple analysis range test. Results: The results showed that the gingival index score levels were decreased significantly in all treatment groups between pre and post treatment at the same group. Conclusions: Indomethacin mouth wash have anti inflammatory effect that can decrease gingival index level.

Abstract In the field of tissue engineering, achieving precise spatiotemporal control over engineered cells is critical for sculpting functional 2D cell cultures into intricate morphological shapes. In this study, we engineer light-responsive mammalian cells and target them with dynamic light patterns to realize 2D cell culture patterning control. To achieve this, we developed μPatternScope (μPS), a modular framework for software-controlled projection of high-resolution light patterns onto microscope samples. μPS comprises hardware and software suite governing pattern projection and microscope maneuvers. Together with a 2D culture of the engineered cells, we utilize μPS for controlled spatiotemporal induction of apoptosis to generate desired 2D shapes. Furthermore, we introduce interactive closed-loop patterning, enabling a dynamic feedback mechanism between the measured cell culture patterns and the light illumination profiles to achieve the desired target patterning trends. Our work offers innovative tools for advanced tissue engineering applications through seamless fusion of optogenetics, optical engineering, and cybernetics.

Abstract Recent advances in tissue engineering have been remarkable, yet the precise control of cellular behavior in 2D and 3D cultures remains challenging. One approach to address this limitation is to genomically engineer optogenetic control of cellular processes into tissues using gene switches that can operate with only a few genomic copies. Here, we implement blue and red light-responsive gene switches to engineer genomically stable two- and three-dimensional mammalian tissue models. Notably, we achieve precise control of cell death and morphogen-directed patterning in 2D and 3D tissues by optogenetically regulating cell necroptosis and synthetic WNT3A signaling at high spatiotemporal resolution. This is accomplished using custom-built patterned LED systems, including digital mirrors and photomasks, as well as laser techniques. These advancements demonstrate the capability of precise spatiotemporal modulation in tissue engineering and open up new avenues for developing programmable 3D tissue and organ models, with significant implications for biomedical research and therapeutic applications.

Abstract To evaluate the effect of combining different bioactive root-end filling materials with composite bone graft (xenogeneic mixed with autogenous bone fragments) on the healing process of periapical tissues after endodontic micro-surgery procedure. In this triple-blinded clinical trial, 56 patients were divided into 2 main groups (28 each) according to the root-end filling material and 2 subgroups according to the presence or absence of the composite bone graft material. Group I: MTA root-end filling (n = 28) in which there are Sub-group A: without bone graft (n = 14) and Sub-group B: with composite bone graft (n = 14). Group II: TotalFill root-end filling (n = 28) in which there are Sub-group A: without bone graft (n = 14) and Sub-group B: with composite bone graft (n = 14). Healthy patients whose ages range from 20 to 50 years with small-to-medium size radiolucency in CBCT related to single-rooted maxillary teeth were included in this study. Patients were assigned a number starting from 1 to 56 and were randomly allocated to four test groups (2 main groups and 2 sub-groups) following simple randomization procedure guidelines described by IBM SPSS V23 (IBM USA) statistical analysis software. This trial was triple-blind where the patient, the outcome assessors, and the main operator were blinded to the applied intervention. Every patient was evaluated clinically and by CBCT at two main observation periods: presurgical and 12-month post-operative. They were also examined and evaluated clinically and radiographically through periapical x-rays after one week, three, and six months. Statistical analysis was performed with IBM SPSS Statistics for Windows Version 23.0. Armonk, NY: IBM Corp. Of the 56 patients enrolled in the study, 49 patients were available for the final analysis. All groups showed no statistically significant differences with regard to healing or success rates at the 12-month follow-up mark. No adverse effects were encountered. Results showed that high success rates were achieved using MTA and TotalFill in the healing of periapical lesions after endodontic surgery. The addition of bone graft in small-to-medium size lesions did not affect the success rate of endodontic surgeries.

Abstract Background The ultimate aim of medical education is to produce successful practitioners, which is a goal that educators, students and stakeholders support. These groups consider success to comprise optimum patient care with consequently positive career progression. Accordingly, identification of the common educational features of such high-achieving doctors will facilitate the generation of clinical excellence amongst future medical trainees. In our study we source data from British clinical merit award schemes and subsequently identify the medical school origins of anaesthetists who have achieved at least national distinction. Methods Britain operates Distinction Award/Clinical Excellence Award schemes which honour National Health Service doctors in Scotland, Wales and England who are identified as high achievers. This quantitative observational study used these awards as an outcome measure in an analysis of the 2019–20 dataset of all 901 national award-winning doctors. Where appropriate, Pearson's Chi-Square test was applied. Results The top five medical schools (London university medical schools, Edinburgh, Dundee, Aberdeen and Glasgow) were responsible for 56.4% of the anaesthetist award-winners, despite the dataset representing 85 medical schools. 93.6% of the anaesthetist merit award-winners were from European medical schools. 8.06% of the anaesthetist award-winners were international medical graduates compared with 11.5% non-anaesthetist award-winners being international medical graduates. Conclusions The majority of anaesthetists who were national merit award-winners originated from only five, apparently overrepresented, UK university medical schools. In contrast, there was a greater diversity of medical school origins among the lower grade national award-winners; tier 3 award-winners represented 20 different medical schools from three continents. As well as ranking educationally successful university medical schools, this study assists UK and international students, by providing a roadmap for rational decision making when selecting anaesthetist and non-anaesthetist medical education pathways that are more likely to fulfil their career ambitions.

This review explores recent advancements in using biomass-derived materials for alkali metal-sulfur and selenium batteries, which are rapidly evolving in the field of high-energy–density storage systems. At the core of our discussion is the utilization of biomass-derived carbon (BDCs), emphasizing its vital role in enhancing the performance of these batteries. We examine the applications of carbon derived from biomass as hosts, extending our exploration beyond lithium-sulfur (Li-S) batteries to include a broader range of alkali metal combinations with sulfur and selenium. We emphasize the rational design and strategic use of biomass-derived materials in addressing challenges such as polysulfide and polyselenide dissolution and slow redox kinetics. The review highlights how these carbon materials contribute to high energy density and long cycling lifespans in sulfur and selenium-based batteries, enhancing stability and efficiency. Concluding with a forward-looking perspective, it identifies the ongoing need for innovation in biomass-derived carbon applications to advance alkali metal-sulfur and selenium batteries’ capabilities.

As load-bearing components, metallic implants are frequently used for orthopedic prostheses due to their superiority over conventional ceramic and polymeric biomaterials. Metal-based orthopedic implants have been subjected to various fabrication and treatment methods to enhance their biological activities at the host site. Modifying the structure, improving the hydrophilicity, and developing controlled-release drug delivery systems can improve cellular adhesion, proliferation, osseointegration, and differentiation. Ultrafast lasers have recently attracted interest in surface engineering. Laser surface structuring permits the alteration of sample topography, the chemical makeup of the surface, and the material physical properties. This study presents the surface modification of a TiNbZrSn shape memory alloy using a femtosecond laser to produce laser-induced periodic structures with better drug release than that of a pristine sample. We also present the in vitro cell viability and biocompatibility of the alloy system. All samples structured with femtosecond laser exhibited superhydrophilic nature with 0° contact angle. In addition, the laser structured surfaces showed cell viability above 80 % and minimal cytotoxicity towards human keratinocytes. Moreover, a well-defined hydroxyapatite layer developed on the laser structured surface. In general, the laser structuring process and the induced changes on the surface in terms of roughness and oxide formation result in slower drug release (up to 10 %) compared to pristine specimens.

Abstract Incorporating selenium into high-surface-area carbon with hierarchical pores, derived from red kidney bean peels via simple carbonization/activation, yields a superior Li–Se battery cathode material. This method produces a carbon framework with 568 m2 g−1 surface area, significant pore volume, and improves the composite’s electronic conductivity and stability by mitigating volume changes and reducing lithium polyselenide dissolution. The Se@ACRKB composite, containing 45 wt% selenium, shows high discharge capacities (609.13 mAh g−1 on the 2nd cycle, maintaining 470.76 mAh g−1 after 400 cycles at 0.2 C, and 387.58 mAh g−1 over 1000 cycles at 1 C). This demonstrates exceptional long-term stability and performance, also applicable to Na–Se batteries, with 421.36 mAh g−1 capacity after 200 cycles at 0.1 C. Our study showcases the potential of using sustainable materials for advanced battery technologies, emphasizing cost-effective and scalable solutions for energy storage.

Abstract Pediatric cardiomyopathies are mostly attributed to variants in sarcomere-related genes. Unfortunately, the genetic architecture of pediatric cardiomyopathies has never been previously studied in Jordan. We sought to uncover the genetic landscape of 14 patients from nine families with several subtypes of pediatric cardiomyopathies in Jordan using Exome sequencing (ES). Our investigation identified pathogenic and likely pathogenic variants in seven out of nine families (77.8%), clustering in sarcomere-related genes. Surprisingly, phenocopies of sarcomere-related hypertrophic cardiomyopathies were evident in probands with glycogen storage disorder and mitochondrial-related disease. Our study underscored the significance of streamlining ES or expanding cardiomyopathy-related gene panels to identify plausible phenocopies of sarcomere-related cardiomyopathies. Our findings also pointed out the need for genetic testing in patients with cardiomyopathy and their at-risk family members. This can potentially lead to better management strategies, enabling early interventions, and ultimately enhancing their prognosis. Finally, our findings provide an initial contribution to the currently absent knowledge about the molecular underpinnings of cardiomyopathies in Jordan.

Thus, the aim of the present study was to develop and evaluate the content validity and acceptance of the multidomain intervention module to reverse cognitive frailty among older adults (iAGELESS).Patients and Methods: This study was conducted in two phases: Phase I included the development of the multidomain intervention module iAGELESS and evaluation of content validity, while Phase II consisted of evaluating the acceptance of the module among 18 healthcare and social care providers, 13 older adults with cognitive frailty, and 13 caregivers. Content validity index (CVI) was used to quantify the content validity. Respondents completed a questionnaire which consisted of information on sociodemographic, followed by module acceptance evaluation with respect to content, terminologies, and graphics. The data was then analyzed descriptively.Results: A multidomain intervention module, iAGELESS was developed. The module was found to have appropriate content validity (overall CVI = 0.83). All the caregivers, 92% of older adults with cognitive frailty and 83% of healthcare and social care providers were satisfied with the overall content of the module. More than 50% of those who accepted the module had satisfactory consensus on the ease of the terminologies, length of sentences, pictures, information, color, and font size included in the module.Conclusion: The iAGELESS module demonstrated good content validity and was well accepted, thus warranting its utilization in future studies to determine its effectiveness in reversing cognitive frailty among older adults.Keywords: aging, cognitive frailty, multidomain intervention module, older adults, reversal

Abstract Central to analyzing noisy gene expression systems is solving the Chemical Master Equation (CME), which characterizes the probability evolution of the reacting species’ copy numbers. Solving CMEs for high-dimensional systems suffers from the curse of dimensionality. Here, we propose a computational method for improved scalability through a divide-and-conquer strategy that optimally decomposes the whole system into a leader system and several conditionally independent follower subsystems. The CME is solved by combining Monte Carlo estimation for the leader system with stochastic filtering procedures for the follower subsystems. We demonstrate this method with high-dimensional numerical examples and apply it to identify a yeast transcription system at the single-cell resolution, leveraging mRNA time-course experimental data. The identification results enable an accurate examination of the heterogeneity in rate parameters among isogenic cells. To validate this result, we develop a noise decomposition technique exploiting time-course data but requiring no supplementary components, e.g., dual-reporters.

Autism is a challenging brain disorder affecting children at global and national scales. Applied behavior analysis is commonly conducted as an efficient medical therapy for children. This paper focused on one paradigm of applied behavior analysis, imitation, where children mimic certain lessons to enhance children’s social behavior and play skills. This paper introduces IMITASD, a practical monitoring assessment model designed to evaluate autistic children’s behaviors efficiently. The proposed model provides an efficient solution for clinics and homes equipped with mid-specification computers attached to webcams. IMITASD automates the scoring of autistic children’s videos while they imitate a series of lessons. The model integrates two core modules: attention estimation and imitation assessment. The attention module monitors the child’s position by tracking the child’s face and determining the head pose. The imitation module extracts a set of crucial key points from both the child’s head and arms to measure the similarity with a reference imitation lesson using dynamic time warping. The model was validated using a refined dataset of 268 videos collected from 11 Egyptian autistic children during conducting six imitation lessons. The analysis demonstrated that IMITASD provides fast scoring, takes less than three seconds, and shows a robust measure as it has a high correlation with scores given by medical therapists, about 0.9, highlighting its effectiveness for children’s training applications.

During recent years, research on authenticated encryption has been thriving through two highly active and practically motivated research directions: provable leakage resilience and key- or context-commitment security. However, the intersection of both fields had been overlooked until very recently. In ToSC 1/2024, Struck and Weishäupl studied generic compositions of encryption schemes and message authentication codes for building committing leakage-resilient schemes. They showed that, in general, Encrypt-then-MAC (EtM) and MAC-then-Encrypt (MtE) are not committing while Encrypt-and-MAC (EaM) is, under plausible and weak assumptions on the components. However, real-world schemes are rarely strict blackbox constructions. Instead, while various leakage-resilient schemes follow blueprints inspired by generic compositions, they often tweak them for security or efficiency. In this paper, we study two blueprints, the first one based on EtM for one of the strongest possible levels of leakage resilience. The second one is a single-pass framework based on leveled implementations. We show that, with a careful selection of the underlying primitives such as with identical encryption and authentication keys and a collision-resistant PRF as the MAC, these blueprints are committing. Our results do not contradict the results by Struck and Weishäupl since we pose more, but practically-motivated, requirements on the components. We demonstrate the practical relevance of our results by showing that our results on those blueprints allow us to easily derive proofs that several state-of-the-art leakage-resilient schemes are indeed committing, including TEDT and its descendants TEDT2 and Romulus-T, as well as the single-pass scheme Triplex.

The cathode in lithium-selenium (Li-Se) batteries has garnered extensive attention owing to its superior specific capacity and enhanced conductivity compared to sulfur. Nonetheless, the adoption and advancement of Li-Se batteries face significant challenges due to selenium’s low reactivity, substantial volume fluctuations, and the shuttle effect associated with polyselenides. Single-atom catalysts (SACs) are under the spotlight for their outstanding catalytic efficiency and optimal atomic utilization. To address the challenges of selenium’s low chemical activity and volume expansion in Li-Se batteries, through electrospun, we have developed a lotus root-inspired carbon nanofiber (CNF) material, featured internal multi-channels and anchored with molybdenum (Mo) single atoms (Mo@CNFs). Mo single atoms significantly enhance the conversion kinetics of selenium (Se), facilitating rapid formation of Li2Se. The internally structured multi-channel CNF serves as an effective host matrix for Se, mitigating its volume expansion during the electrochemical process. The resulting cathode, Se/Mo@CNF composite, exhibits a high discharge specific capacity, superior rate performance, and impressive cycle stability in Li-Se batteries. After 500 cycles at a current density of 1 C, it maintains a capacity retention rate of 82% and nearly 100% coulombic efficiency (CE). This research offers a new avenue for the application of single-atom materials in enhancing advanced Li-Se battery performance.

Several factors impact the durability of concrete bridge decks, including traffic loads, fatigue, temperature changes, environmental stress, and maintenance activities. Detecting problems such as corrosion, delamination, or concrete degradation early on can lower maintenance costs. Nondestructive evaluation (NDE) techniques can detect these issues at early stages. Each NDE method, meanwhile, has limitations that reduce the accuracy of the assessment. In this study, multiple NDE technologies were combined with machine learning algorithms to improve the interpretation of half-cell potential (HCP) and electrical resistivity (ER) measurements. A parametric study was performed to analyze the influence of five parameters on HCP and ER measurements, such as the degree of saturation, corrosion length, delamination depth, concrete cover, and moisture condition of delamination. The results were obtained through finite element simulations and used to build two machine learning algorithms, a classification algorithm and a regression algorithm, based on Random Forest methodology. The algorithms were tested using data collected from a bridge deck in the BEAST® facility. Both machine learning algorithms were effective in improving the interpretation of the ER and HCP measurements using data from multiple NDE technologies.

Abstract Background Dual-energy X-ray absorptiometry (DXA) is an important diagnostic test for bone mass status. The aim of this work was to set the standards for structured reporting of DXA measurements in adults within the context of fracture and fall risk assessment. Results Two rounds of Delphi were completed. The first Delphi round had a 68% response rate, while round two had a 100% response rate. After round 2, a total of 28 items were obtained, which were classified into three domains. The percentage of people who agreed with the recommendations (ranks 9–7) ranged from 76.5 to 100%. The wording of all 19 clinical standards determined by the scientific committee was agreed upon (i.e., 75% of respondents strongly agreed or agreed). Conclusion The DXA scan report is an independent document that contains sufficient information to enable optimal osteoporosis management advised by an experienced healthcare professional. Setting up quality standards for DXA scans not only supports healthcare professionals reporting/interpreting bone densitometry but also meets the parameters outlined in national as well as international guidelines or recommendations for the optimal management of osteoporosis and subsequent prevention of low trauma fractures.

Introduction, A decade ago, stereotactic radiosurgery (SRS) without whole brain radiotherapy (WBRT) was emerging as preferred treatment for oligometastatic brain metastases. Studies of cavity SRS after neurosurgery were underway. Data specific to metastatic HER2 breast cancer (MHBC), describing intracranial, systemic and survival outcomes without WBRT, were lacking. A Phase II study was designed to address this gap.Method, Adults with MHBC, performance status 0–2, ≤ five BrM, receiving/planned to receive HER2-targeted therapy were eligible. Exclusions included leptomeningeal disease and prior WBRT. Neurosurgery allowed ≤6 weeks before registration and required for BrM >4 cm. Primary endpoint was 12-month requirement for WBRT. Secondary endpoints; freedom from (FF-) local failure (LF), distant brain failure (DBF), extracranial disease failure (ECDF), overall survival (OS), cause of death, mini-mental state examination (MMSE), adverse events (AE).Results, Twenty-five patients accrued Decembers 2016–2020. The study closed early after slow accrual. Thirty-seven BrM and four cavities received SRS. Four cavities and five BrM were observed. At 12 months: one patient required WBRT (FF-WBRT 95 %, 95 % CI 72–99), FFLF 91 % (95 % CI 69–98), FFDBF 57 % (95 % CI 34–74), FFECDF 64 % (95 % CI 45–84), OS 96 % (95 % CI 74–99). Two grade 3 AE occurred. MMSE was abnormal for 3/24 patients at baseline and 1/17 at 12 months.Conclusion, At 12 months, SRS and/or neurosurgery provided good control with low toxicity. WBRT was not required in 95 % of cases. This small study supports the practice change from WBRT to local therapies for MHBC BrM.

Abstract Background The clinical heterogeneity of chronic rhinosinusitis (CRS) and bronchial asthma is attributable to different underlying inflammatory profiles. However, the similarity between CRS with nasal polyps (CRSwNP) and type-2 asthma pathophysiology speculates that one biological therapy could affect both comorbidities. Despite dupilumab, a monoclonal antibody that targets IL-4α and IL-13 receptors, being used in patients with nasal polyps and severe asthma, real-life data about its efficacy in improving the quality of life and patient symptoms is still lacking. This study’s primary objective was to evaluate dupilumab treatment’s effect on the frequency of olfactory symptoms and health-related quality of life tests as measured by the Sino-nasal outcome test (SNOT-22) in patients with NP. The secondary objective was the effect of dupilumab on asthma symptom control as measured by the asthma control test (ACT). Methods A prospective study was conducted of 166 patients with CRSwNP, with or without asthma. The following variables were collected at baseline and after at least six months of continuous dupilumab therapy; SNOT-22, olfactory symptoms frequency, and ACT score. Results Asthma prevalence in patients with CRSwNP was high (59.63%), and being female with a history of frequent use of oral corticosteroid (OCS) courses and repeated unsuccessful nasal and para-nasal surgeries for polyposis increased the likelihood of having underlying asthma by 2, 1 and 4 times more, respectively. Additionally, being asthmatic required a longer duration of dupilumab treatment. However, both the health-related quality of life and olfactory symptoms improved equally in both groups. Conclusion Even with associated comorbid asthma in patients with CRSwNP, treatment with dupilumab could improve the quality of life, olfactory symptoms, and asthma symptom control.

IntroductionThe American Urological Association (AUA) and the European Association of Urology (EAU) currently recommend partial nephrectomy (PN) over ablation for cT1b lesions. However, recent series have shown comparable outcomes for cryoablation (CA) when compared to PN, making it an appealing alternative for a select group of patients. The objective of this manuscript is to assess treatment outcomes and complications of CA for cT1b lesions. MethodsUsing Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) guidelines, a comprehensive search was done on MEDLINE and Cochrane Library electronic databases identifying studies that reported on outcomes and complications of CA for kidney tumors. Inclusion criteria included cT1b lesions between 4 cm and 7 cm, excluding treatment of other sizes. ResultsA total of 347 patients with cT1b lesions identified on imaging underwent percutaneous or laparoscopic CA. The average age was > 65 years, the median size of lesions and RENAL score ranged between 4.3–4.8 cm and 8–9, respectively. The majority of patients had a Charlson comorbidity index (CCI) of 2, and median follow-up ranged between 13 months and 95 months. Across all the series, primary and secondary success rates were between 84%–98% and 92%–98%, respectively. The local recurrence ranged from 2.8% to 27%. For patients with documented RCC on biopsy, the 5-year overall survival (OS), cancer-specific survival (CSS), recurrence-free survival (RFS), and metastasis-free survival (MFS) ranged from 56%–91%, 85%–100%, 70%–96.4%, and 90%–96%, respectively. The major complication rate (Clavien-Dindo III-V) was low, at 6.2%. ConclusionWith promising survival outcomes and low complication rates perioperatively, CA is acceptable in a select group of patients with T1b renal tumors, including those who are older, have multiple comorbidities, or have relative or absolute contraindication to surgery.

تهدف هذه الدراسة الى إمكان تطبيق القواعد العامة على القوانين الخاصة، والمقصد من ذلك اي حال عدم توفر نص في القانون الخاص إجرائي او موضوعي ووقعت جريمة الكترونية وفقاً لقانون الجرائم الإلكترونية الاردني رقم 17 لسنة 2023، فهل يمكن تطبيق النصوص الواردة في القوانين الأخرى العامة كقانون أصول المحاكمات الجزائية الأردني او قانون العقوبات الأردني او اي من القوانين ذات العلاقة، وهي ذات ارتباط في قانون الجرائم الإلكترونية، بإعتبار ان الجريمة وقعت بصورة الكترونية؟. وقد امتد هدف هذه الدراسة الى جعل الجريمة الإلكترونية نموذج لها، بإعتبار انه قد جاء قانون جديد لا يشابه القانون السابق في المملكة الأردنية الهاشمية، الذي عانت معه التطبيقات القضائية في بحث إمكان تطبيق القواعد العامة على القوانين الخاصة بسبب قصور ذلك القانون. وقد وجدت الدراسة بعد التمحيص والتدقيق على إمكان تطبيق القوواعد العامة على القوانين الخاصة ولكن ضمن حدود معينة لا يمكن معها الخروج عن المألوف او التعديد على النصوص الواردة في القوانين الخاصة. من ذلك، خرج الباحث بجملة من الآراء والإجتهادات القضائية التي تساعد على ضبط النقص التشريعي في القوانين الخاصة ومنها قانون الجرائم الإلكترونية وإمكان تطبيق القواعد الخاصة للوصول الى العدالة المبلجة.

Background/objective: Given high mortality rates, understanding the management of associated hypothermia and hyponatremia in the myxedema coma remains crucial. Case report: A 69-year-old Caucasian male from New England with no known medical history presented to the hospital after a mechanical fall. He was lethargic but arousable on presentation. The vitals showed a rectal temperature of 84.9 °F, heart rate of 48 beats/minute, and blood pressure of 139/87 mmHg. He had delayed reflexes. Active rewarming was initiated to treat hypothermia which led to hypotensive episodes. Initial studies showed TSH of 52 uIU/ML (0.35–5.5), undetectable free T4, and he was treated with intravenous stress-dose glucocorticoids and levothyroxine. Hypotension was partially responsive to fluid therapy but required norepinephrine infusion later. Septic and cardiogenic shock were ruled out retrospectively.He had serum sodium of 117 MEQ/L, serum osmolality of 255 mOSM/KG), urine sodium of 29 MEQ/L, and urine osmolality of 529 mOSM/KG. Hyponatremia improved with levothyroxine replacement and fluid restriction, suggestive of an SIADH-like syndrome. Further studies revealed positive thyroid peroxidase and thyroglobulin antibodies. A neck ultrasound revealed a partially visualized atrophic thyroid gland. Discussion: Active rewarming to treat hypothermia in myxedema can lead to hemodynamic instability. Studies report a weak association between hypothyroidism and hyponatremia. Severe hyponatremia from an SIADH-like syndrome can occur in myxedema. Conclusion: Avoid active rewarming in myxedema. Hyponatremia from an SIADH-like syndrome in myxedema coma corrects with fluid restriction and levothyroxine replacement.

Chemical additives to perovskite precursors constitute a predominant route to passivate defects in perovskite films and improve carrier charge dynamics. In this work, ionic liquids (ILs) with chloride and iodide anions are used as additives for bulk passivation of perovskite films. The ionic liquids in the form of 1‑butyl‑4-methylpyradinium iodide (IL-I) and 1-ethylimidazolium chloride (IL-Cl) are mixed individually with a precursor of Cesium Formamidinium Lead Triiodide and Methylammonium Chloride (CsFAPbI3−MACl). The decay of charge carriers within the perovskite film before and after passivation with ILs was measured by Time Resolved Photoluminescence Spectroscopy (TRPL). The effect of added ILs on the recombination rate of the photocarriers was investigated by analyzing the decay of the TRPL signal in all samples. The average lifetimes obtained within one day of preparations were 79 ns, 73 ns and 275 ns for pristine perovskite, IL-I passivated perovskite and IL-Cl passivated perovskite films, respectively. These values show that the presence of the IL with chloride anion greatly improves the average lifetime of the charge carrier by around 250%, while IL with iodide anion decreased the lifetime by 32% as compared with the pristine perovskite sample. This finding renders the IL-Cl passivated perovskite films as a valuable resource in applications ranging from photoelectric to hydrogen generation devices.

Abstract Leptomeningeal metastases (LM) are a devastating complication of HER2 + metastatic breast cancer (MBC), with no effective treatments. In a case series of 8 patients with heavily pretreated HER2 + MBC and progressing LM, all 8 patients (100%) derived clinical benefit from Trastuzumab deruxtecan (TDXd), and 4 patients (50%) had an objective partial response based on formal neuroradiology MRI reads using the EORTC/RANO-LM Revised-Scorecard. T-DXd warrants further study in LM in HER2 + MBC and solid tumors where T-DXd may be active.

Homeostasis and robust perfect adaptation are remarkable features of living cells. Here, to synthetically achieve this, the authors present a theoretical and experimental framework using inteins to implement compact biomolecular integral feedback controllers.

The well-known Internal Model Principle (IMP) is a cornerstone of modern control theory. It stipulates the necessary conditions for asymptotic robustness of disturbance-prone dynamical systems by asserting that such a system must embed a subsystem in a feedback loop, and this subsystem must be able to reduplicate the dynamic disturbance using only the regulated variable as the input. The insights provided by IMP can help in both designing suitable controllers and also in analysing the regulatory mechanisms in complex systems. So far the application of IMP in biology has been case-specific and ad hoc, primarily due to the lack of generic versions of the IMP for biomolecular reaction networks that model biological processes. In this short article we highlight the need for an IMP in biology and discuss a recently developed version of it for biomolecular networks that exhibit maximal Robust Perfect Adaptation (maxRPA) by being robust to the maximum number of disturbance sources.

Summary: Recent progress in protein engineering has established optogenetics as one of the leading external non-invasive stimulation strategies, with many optogenetic tools being designed for in vivo operation. Characterization and optimization of these tools require a high-throughput and versatile light delivery system targeting micro-titer culture volumes. Here, we present a universal light illumination platform – Diya, compatible with a wide range of cell culture plates and dishes. Diya hosts specially designed features ensuring active thermal management, homogeneous illumination, and minimal light bleedthrough. It offers light induction programming via a user-friendly custom-designed GUI. Through extensive characterization experiments with multiple optogenetic tools in diverse model organisms (bacteria, yeast, and human cell lines), we show that Diya maintains viable conditions for cell cultures undergoing light induction. Finally, we demonstrate an optogenetic strategy for in vivo biomolecular controller operation. With a custom-designed antithetic integral feedback circuit, we exhibit robust perfect adaptation and light-controlled set-point variation using Diya.

Abstract Topological mechanical metamaterials unlock confined and robust elastic wave control. Recent breakthroughs have precipitated the development of 3D topological metamaterials, which facilitate extraordinary wave manipulation along 2D planar and layer‐dependent waveguides. The 3D topological metamaterials studied thus far are constrained to function in single‐frequency bandwidths that are typically in a high‐frequency regime, and a comprehensive experimental investigation remains elusive. In this paper, these research gaps are addressed and the state of the art is advanced through the synthesis and experimental realization of a 3D topological metamaterial that exploits multimodal local resonance to enable low‐frequency elastic wave control over multiple distinct frequency bands. The proposed metamaterial is geometrically configured to create multimodal local resonators whose frequency characteristics govern the emergence of four unique low‐frequency topological states. Numerical simulations uncover how these topological states can be employed to achieve polarization‐, frequency‐, and layer‐dependent wave manipulation in 3D structures. An experimental study results in the attainment of complete wave fields that illustrate 2D topological waveguides and multi‐polarized wave control in a physical testbed. The outcomes from this work provide insight that will aid future research on 3D topological mechanical metamaterials and reveal the applicability of the proposed metamaterial for wave control applications.

IntroductionImproving maternal health and survival remains a public health priority for Sudan. Significant investments were made to expand access to maternal health services, such as through the training and deployment of providers with varying skills and competencies to work across the country. This study investigates trends in the coverage of different birth attendants and their relationship with the maternal mortality ratio (MMR).MethodsTrend analyses were conducted using data from the 2006, 2010, and 2014 Sudan Household surveys. Three categories of birth attendants were identified: (1) skilled birth attendants (SBA) such as doctors, nurse-midwives, and health visitors, (2) locally certified midwives, and (3) traditional birth attendants (TBA). Multivariable logistic regression models were used to examine trends in SBAs (vs. locally certified midwives and TBAs), locally certified midwives (vs SBAs and TBAs), and SBAs and locally certified midwives by place of birth (health facility and home). The analyses were adjusted for potential confounders. An ecological analysis was conducted to assess the relationship between birth attendants by place of birth and MMR at the state level.ResultsBirths by 15,848 women were analysed. Locally certified midwives attended most births in each survey year, with their contribution increasing from 36.3% in 2006 to 55.5% in 2014. The contributions of SBAs and TBAs decreased over the same period. In 2014 compared with 2006, births were more likely to be attended by a locally certified midwife (aOR: 2.19; 95%CI: 1.82–2.63) but less likely to be attended by a SBA (aOR: 0.46; 95%CI: 0.37–0.56). The decrease in SBA was more substantial for births taking place at home (aOR: 0.17; 95%CI: 0.12–0.23) than for health facility births (aOR: 0.45; 95%CI: 0.31–0.65). In the ecological analysis 2014–2016, the proportion of births attended by SBA in health facilities correlated negatively with MMR at state level (rho −0.55; p: 0.02).ConclusionThis analysis suggests that although an improved coverage of maternal health with locally certified midwives has been observed, it has not provided the skill level reached by SBA. SBAs working in facility settings were a key correlating factor to reduced maternal mortality. Urgent action is needed to improve access to SBAs in health facilities, thereby accelerating progress in reducing maternal mortality.

Introduction Glioblastoma is the most common aggressive primary central nervous system cancer in adults characterised by uniformly poor survival. Despite maximal safe resection and postoperative radiotherapy with concurrent and adjuvant temozolomide-based chemotherapy, tumours inevitably recur. Imaging with O-(2-[18F]-fluoroethyl)-L-tyrosine (FET) positron emission tomography (PET) has the potential to impact adjuvant radiotherapy (RT) planning, distinguish between treatment-induced pseudoprogression versus tumour progression as well as prognostication.Methods and analysis The FET-PET in Glioblastoma (FIG) study is a prospective, multicentre, non-randomised, phase II study across 10 Australian sites and will enrol up to 210 adults aged ≥18 years with newly diagnosed glioblastoma. FET-PET will be performed at up to three time points: (1) following initial surgery and prior to commencement of chemoradiation (FET-PET1); (2) 4 weeks following concurrent chemoradiation (FET-PET2); and (3) within 14 days of suspected clinical and/or radiological progression on MRI (performed at the time of clinical suspicion of tumour recurrence) (FET-PET3). The co-primary outcomes are: (1) to investigate how FET-PET versus standard MRI impacts RT volume delineation and (2) to determine the accuracy and management impact of FET-PET in distinguishing pseudoprogression from true tumour progression. The secondary outcomes are: (1) to investigate the relationships between FET-PET parameters (including dynamic uptake, tumour to background ratio, metabolic tumour volume) and progression-free survival and overall survival; (2) to assess the change in blood and tissue biomarkers determined by serum assay when comparing FET-PET data acquired prior to chemoradiation with other prognostic markers, looking at the relationships of FET-PET versus MRI-determined site/s of progressive disease post chemotherapy treatment with MRI and FET-PET imaging; and (3) to estimate the health economic impact of incorporating FET-PET into glioblastoma management and in the assessment of post-treatment pseudoprogression or recurrence/true progression. Exploratory outcomes include the correlation of multimodal imaging, blood and tumour biomarker analyses with patterns of failure and survival.Ethics and dissemination The study protocol V.2.0 dated 20 November 2020 has been approved by a lead Human Research Ethics Committee (Austin Health, Victoria). Other clinical sites will provide oversight through local governance processes, including obtaining informed consent from suitable participants. The study will be conducted in accordance with the principles of the Declaration of Helsinki and Good Clinical Practice. Results of the FIG study (TROG 18.06) will be disseminated via relevant scientific and consumer forums and peer-reviewed publications.Trial registration number ANZCTR ACTRN12619001735145

Background and Objectives: Human papillomavirus (HPV) was previously investigated in lung cancer with wide inter-geographic discrepancies. p16INK4a has been used as a surrogate for detecting high-risk HPV (HR-HPV) in some cancer types. This study assessed the evidence of HPV in non-small-cell lung cancer (NSCLC) among Jordanian patients, investigated the expression of p16INK4a, and evaluated its prognostic value and association with HPV status. Materials and Methods: The archived samples of 100 patients were used. HPV DNA detection was performed by real-time polymerase chain reaction (RT-PCR). p16INK4a expression was assessed by immunohistochemistry (IHC). The Eighth American Joint Committee on Cancer protocol (AJCC) of head and neck cancer criteria were applied to evaluate p16INK4a positivity considering a moderate/strong nuclear/cytoplasmic expression intensity with a distribution in ≥75% of cells as positive. Results: HPV DNA was detected in 5% of NSCLC cases. Three positive cases showed HR-HPV subtypes (16, 18, 52), and two cases showed the probable HR-HPV 26 subtype. p16INK4a expression was positive in 20 (20%) NSCLC cases. None of the HPV-positive tumors were positive for p16INK4a expression. A statistically significant association was identified between p16INK4a expression and the pathological stage (p = 0.029) but not with other variables. No survival impact of p16INK4a expression was detected in NSCLC cases as a group; however, it showed a statistically significant association with overall survival (OS) in squamous cell carcinoma (SqCC) cases (p = 0.033). Conclusions: This is the first study to assess HPV and p16INK4a expression in a Jordanian population. HPV positivity is rare in NSCLC among a Jordanian subpopulation. P16 INK4a reliability as a surrogate marker for HPV infection in lung cancer must be revisited.

Responses to immune checkpoint blockade (ICB) in patients with glioblastoma are limited. Here the authors show that Calmodulin-Dependent Kinase Kinase 2 (CaMKK2) is expressed in tumor associated macrophages and neurons and is associated with resistance to ICB in preclinical models of glioblastoma.

Communities of microbes play important roles in natural environments and hold great potential for deploying division-of-labor strategies in synthetic biology and bioproduction. Here, in a community of two competing E. coli strains, the authors show that the relative abundances of the strains can be stabilized and steered dynamically with remarkable precision by coupling the cells to an automated computer-controlled feedback-loop.

Backgroundprecision-medicine quantitative tools for cross-sectional imaging require painstaking labeling of targets that vary considerably in volume, prohibiting scaling of data annotation efforts and supervised training to large datasets for robust and generalizable clinical performance. A straight-forward time-saving strategy involves manual editing of AI-generated labels, which we call AI-collaborative labeling (AICL). Factors affecting the efficacy and utility of such an approach are unknown. Reduction in time effort is not well documented. Further, edited AI labels may be prone to automation bias.PurposeIn this pilot, using a cohort of CTs with intracavitary hemorrhage, we evaluate both time savings and AICL label quality and propose criteria that must be met for using AICL annotations as a high-throughput, high-quality ground truth.Methods57 CT scans of patients with traumatic intracavitary hemorrhage were included. No participant recruited for this study had previously interpreted the scans. nnU-net models trained on small existing datasets for each feature (hemothorax/hemoperitoneum/pelvic hematoma; n = 77–253) were used in inference. Two common scenarios served as baseline comparison- de novo expert manual labeling, and expert edits of trained staff labels. Parameters included time effort and image quality graded by a blinded independent expert using a 9-point scale. The observer also attempted to discriminate AICL and expert labels in a random subset (n = 18). Data were compared with ANOVA and post-hoc paired signed rank tests with Bonferroni correction.ResultsAICL reduced time effort 2.8-fold compared to staff label editing, and 8.7-fold compared to expert labeling (corrected p

Objectives To explore informal caregivers’ perspectives on precision medicine in cancer care.Design Semi-structured interviews with the informal caregivers of people living with cancer and receiving targeted/immunotherapies. Interview transcripts were analysed thematically using a framework approach.Setting Recruitment was facilitated by two hospitals and five Australian cancer community groups.Participants Informal caregivers (n=28; 16 men, 12 women; aged 18–80) of people living with cancer and receiving targeted/immunotherapies.Results Thematic analysis identified three findings, centred largely on the pervasive theme of hope in relation to precision therapies including: (1) precision as a key component of caregivers’ hope; (2) hope as a collective practice between patients, caregivers, clinicians and others, which entailed work and obligation for caregivers; and (3) hope as linked to expectations of further scientific progress, even if there may be no personal, immediate benefit.Conclusions Innovation and change in precision oncology are rapidly reconfiguring the parameters of hope for patients and caregivers, creating new and difficult relational moments and experiences in everyday life and in clinical encounters. In the context of a shifting therapeutic landscape, caregivers’ experiences illustrate the need to understand hope as collectively produced, as emotional and moral labour, and as entangled in broader cultural expectations of medical advances. Such understandings may help clinicians as they guide patients and caregivers through the complexities of diagnosis, treatment, emerging evidence and possible futures in the precision era. Developing a better understanding of informal caregivers’ experiences of caring for patients receiving precision therapies is important for improving support to patients and their caregivers.

The invention of the Fourier integral in the 19th century laid the foundation for modern spectral analysis methods. Here the authors develop frequency-based methods for analyzing the reaction mechanisms within living cells from distinctively noisy single-cell output trajectories and present forward engineering of synthetic oscillators and controllers.

Selenium (Se)-based cathode materials have garnered considerable interest for lithium-ion batteries due to their numerous advantages, including low cost, high volumetric capacity (3268 mAh cm−3), high density (4.82 g cm−3), ability to be cycled to high voltage (4.2 V) without failure, and environmental friendliness. However, they have low electrical conductivity, low coulombic efficiency, and polyselenide solubility in electrolytes (shuttle effect). These factors have an adverse effect on the electrochemical performance of Li-Se batteries, rendering them unsuitable for real-world use. In this study, we briefly examined numerous approaches to overcoming these obstacles, including selecting an adequate electrolyte, the composition of Se with carbonaceous materials, and the usage of metal selenide base electrodes. Furthermore, we examined the effect of introducing interlayers between the cathode and the separator. Finally, the remaining hurdles and potential study prospects in this expanding field are proposed to inspire further insightful work.

Abstract Combining single‐cell measurements of ERK activity dynamics with perturbations provides insights into the MAPK network topology. We built circuits consisting of an optogenetic actuator to activate MAPK signaling and an ERK biosensor to measure single‐cell ERK dynamics. This allowed us to conduct RNAi screens to investigate the role of 50 MAPK proteins in ERK dynamics. We found that the MAPK network is robust against most node perturbations. We observed that the ERK‐RAF and the ERK‐RSK2‐SOS negative feedback operate simultaneously to regulate ERK dynamics. Bypassing the RSK2‐mediated feedback, either by direct optogenetic activation of RAS, or by RSK2 perturbation, sensitized ERK dynamics to further perturbations. Similarly, targeting this feedback in a human ErbB2‐dependent oncogenic signaling model increased the efficiency of a MEK inhibitor. The RSK2‐mediated feedback is thus important for the ability of the MAPK network to produce consistent ERK outputs, and its perturbation can enhance the efficiency of MAPK inhibitors.

Objective: It is important to minimize the risk of major vascular injury during pneumoperitoneum establishment in laparoscopic surgeries for patients with unusual variations in the levels of the abdominal aorta, the inferior vena cava (IVC), and the left renal vein, which will decrease the morbidity and mortality. The study aims to assess the variations regarding the bifurcation level of the abdominal aorta, formation level of the IVC, and insertion level of the left renal vein into the IVC. Methods: This retrospective study was conducted on 100 patients (50 males and 50 females) referred to the Department of Radiology, Jordan University Hospital for abdomino-pelvic CT with intra-venous contrast from January 2018 to December 2019. The three vessels were determined on the axial plane, the coronal plane, and the midsagittal plane. The central vertebral body height as well as the distance of the level of the point of interest to the upper end plate of the vertebrae were measured. Afterwards, the results were classified into the following categories, upper end plate, lower end plate, intervertebral disc, upper half, and lower half of the vertebra. Results: The aortic bifurcation was mainly found at the level of the L4 vertebral body (65, 65%). In the remaining cases, the bifurcation was found to be variably located spanning from L3 in 11 (11%) cases to 3 (3%) cases at L5. As for the iliocaval junction, the most common site was also at the level of L4 with 41 (41%) cases followed by 39 (39%) cases at the level of L5, and 20 (20%) cases at the intervertebral disc of L4/L5. The left renal vein most commonly joined the IVC at the level of L1 with 62 (62%) cases followed by 20 (20%) cases at the intervertebral disc T12/L1. There was wide variation in its entry to the IVC spanning from 4 (4%) cases at T12/L1 to 1 (1%) case at L4. Conclusion: The anatomical variation of the major vessels can be found in the normal population. Therefore, sufficient investigation of the anatomical position of these vessels is essential for patients before laparoscopic surgery.

Image-based techniques have become integral to the construction sector, aiding in project planning, progress monitoring, quality control, and documentation. In this paper, we address two key challenges that limit our ability to fully exploit the potential of images. The first is the “semantic gap” between low-level image features and high-level semantic descriptions. The second is the lack of principled integration between images and other digital systems used in construction, such as construction schedules and building information modeling (BIM). These challenges make it difficult to effectively incorporate images into digital twins of construction (DTC), a critical concept that addresses the construction industry’s need for more efficient project management and decision-making. To address these challenges, we first propose an ontology-based construction image interpretation (CII) framework to formalize the interpretation and integration workflow. Then, the DiCon-SII ontology is developed to provide a formalized vocabulary for visual construction contents and features. DiCon-SII also acts as a bridge between images and other digital systems to help construct an image-involved DTC. To evaluate the practical application of DiCon-SII and CII in supporting construction management tasks and as a precursor to DTC, we conducted a case study involving drywall installation. Via this case study, we demonstrate how the proposed methods can be used to infer the operational stage of a construction process, estimate labor productivity, and retrieve specific images based on user queries.

The design of feedback biomolecular controllers is essential to synthetically regulate biological processes in a robust and timely fashion. Here the authors introduce a wide array of biomolecular Proportional-Integral-Derivative (PID) controllers that are capable of enhancing stability and dynamic performance, and also reducing stochastic noise.

Objective To evaluate the effect of colchicine and high-intensity rosuvastatin in addition to standard of care on the progression of COVID-19 disease in hospitalised patients.Design A pragmatic, open-label, multicentre, randomised controlled trial conducted from October 2020 to September 2021. Follow-up was conducted at 30 and 60 days. The electronic medical record was used at all stages of the trial including screening, enrolment, randomisation, event ascertainment and follow-up.Setting Four centres in the Yale New Haven Health System.Participants Non-critically ill hospitalised patients with COVID-19.Interventions Patients were randomised 1:1 to either colchicine plus high-intensity rosuvastatin in addition to standard of care versus standard of care alone. Assigned treatment was continued for the duration of index hospitalisation or 30 days, whichever was shorter.Primary and secondary outcome measures The prespecified primary endpoint was progression to severe COVID-19 disease (new high-flow or non-invasive ventilation, mechanical ventilation, need for vasopressors, renal replacement therapy or extracorporeal membrane oxygenation, or death) or arterial/venous thromboembolic events (ischaemic stroke, myocardial infarction, deep venous thrombosis or pulmonary embolism) evaluated at 30 days.Results Among the 250 patients randomised in this trial (125 to each arm), the median age was 61 years, 44% were women, 15% were Black and 26% were Hispanic/Latino. As part of the standard of care, patients received remdesivir (87%), dexamethasone (92%), tocilizumab (18%), baricitinib (2%), prophylactic/therapeutic anticoagulation (98%) and aspirin (91%). The trial was terminated early by the data and safety monitoring board for futility. No patients were lost to follow-up due to electronic medical record follow-up. There was no significant difference in the primary endpoint at 30 days between the active arm and standard of care arm (15.2% vs 8.8%, respectively, p=0.17).Conclusions In this small, open-label, randomised trial of non-critically ill hospitalised patients with COVID-19, the combination of colchicine and rosuvastatin in addition to standard of care did not appear to reduce the risk of progression of COVID-19 disease or thromboembolic events, although the trial was underpowered due to a lower-than-expected event rate. The trial leveraged the power of electronic medical records for efficiency and improved follow-up and demonstrates the utility of incorporating electronic medical records into future trials.Trial registration NCT04472611.

Aim: This study aimed to investigate the conditioning effects of phosphoric acid/5.25% sodium hypochlorite (NaOCl) mixture, 2% chlorhexidine (CHX) digluconate, and 10% polyacrylic acid on cavosurface microleakage and bond strength of glass ionomer (GI) restorations. Materials and Methods: Out of 68 extracted premolars, 34 teeth were selected for microleakage and 34 for bond strength evaluation. The samples were divided into the following four groups. Group 1: pretreatment with 50/50 volume% mixture of 5.25% NaOCl solution and 37% phosphoric acid (H3PO4), Group 2:pretreatment with 2% CHX digluconate, Group 3: pretreatment with 10% polyacrylic acid (positive control), and Group 4: no pretreatment (negative control). All the samples were then restored with glass ionomer cement (GIC). Microleakage was evaluated using a stereomicroscope and rhodamine-B dye penetration test. For bond strength, flat dentin surface was exposed and pretreated as mentioned previously and restored with GIC and was evaluated using universal testing machine. Results: Among all the four groups, Group 1 showed least microleakage and highest bond strength when compared with other groups. Whereas the Group 4 samples which were not pretreated with any of the conditioning agent showed the least shear bond strength with greatest cavosurface microleakage when compared to the groups which were pretreated with the conditioning agents. Conclusions: A combination of 50/50 volume % mixture of 37% H3PO4 and 5.25% NaOCl can be a good choice for surface pretreatment of GI restorations.

Objective: The present trial aimed to assess the effectiveness of the structured Information, Education, and Communication provided by trained health educators at primary care on the mean body mass index, waist circumference, fasting blood glucose, and the level of blood pressure, among patients with type 2 diabetes mellitus. Research Methods: This was a 12-month pragmatic clustered randomized trial where 180 patients with type 2 diabetes mellitus were enrolled in 2020. Five primary health care centres were randomly allocated to either the intervention or control arm. Results: There was a comparable significant reduction in the mean scores of waist circumference among participants in the intervention versus control arm over 12 months: I =108.7:C = 85.8 cm (P value = 0.001), I = 109.2:C = 86.02 cm (P value = 0.001), and I = 105.6:C = 87.2 cm (P value = 0.001) in the first, sixth, and twelfth months, respectively. A significant reduction in the mean scores of body mass index in the intervention versus control arm only in the first month: I =31.7:C = 26.5 cm (P value = 0.001). However, there was a comparable significant reduction in the mean scores of fasting blood glucose in the intervention versus control arm over 12 months: I =2016.5:C = 185.3 mmol/l (P value = 0.011), I = 207.4:C = 168.04 mmol/l (P value = 0.002), and I = 2012.7:C = 158.6 mmol/l (P value = 0.001) in the first, sixth, and twelfth months, respectively. Conclusions: The delivery of structured diabetes self-care education for diabetics by trained health educators at primary health care has a beneficiary effect on reducing the mean body mass index, waist circumference, and blood glucose. Similarly, it decreases high blood pressure.

Generally, cloud computing is integrated with wireless sensor network to enable the monitoring systems and it improves the quality of service. The sensed patient data are monitored with biosensors without considering the patient datatype and this minimizes the work of hospitals and physicians. Wearable sensor devices and the Internet of Medical Things (IoMT) have changed the health service, resulting in faster monitoring, prediction, diagnosis, and treatment. Nevertheless, there have been difficulties that need to be resolved by the use of AI methods. The primary goal of this study is to introduce an AI-powered, IoMT telemedicine infrastructure for E-healthcare. In this paper, initially the data collection from the patient body is made using the sensed devices and the information are transmitted through the gateway/Wi-Fi and is stored in IoMT cloud repository. The stored information is then acquired, preprocessed to refine the collected data. The features from preprocessed data are extracted by means of high dimensional Linear Discriminant analysis (LDA) and the best optimal features are selected using reconfigured multi-objective cuckoo search algorithm (CSA). The prediction of abnormal/normal data is made by using Hybrid ResNet 18 and GoogleNet classifier (HRGC). The decision is then made whether to send alert to hospitals/healthcare personnel or not. If the expected results are satisfactory, the participant information is saved in the internet for later use. At last, the performance analysis is carried so as to validate the efficiency of proposed mechanism.

Implementation-based attacks are major concerns for modern cryptography. For symmetric-key cryptography, a significant amount of exploration has taken place in this regard for primitives such as block ciphers. Concerning symmetric-key operating modes, such as Authenticated Encryption with Associated Data (AEAD), the stateof-the-art mainly addresses the passive Side-Channel Attacks (SCA) in the form of leakage resilient cryptography. So far, only a handful of work address Fault Attacks (FA) in the context of AEADs concerning the fundamental properties – integrity and confidentiality. In this paper, we address this gap by exploring mode-level issues arising due to FAs. We emphasize that FAs can be fatal even in cases where the adversary does not aim to extract the long-term secret, but rather tries to violate the basic security requirements (integrity and confidentiality). Notably, we show novel integrity attack examples on state-of-the-art AEAD constructions and even on a prior fault-resilient AEAD construction called SIV$. On the constructive side, we first present new security notions of fault-resilience, for PRF (frPRF), MAC (frMAC) and AEAD (frAE), the latter can be seen as an improved version of the notion introduced by Fischlin and Gunther at CT-RSA’20. Then, we propose new constructions to turn a frPRF into a fault-resilient MAC frMAC (hash-then-frPRF) and into a fault-resilient AEAD frAE (MAC-then-Encrypt-then-MAC or MEM).
