Many of the "signs and tokens" described in alchemical texts relate to colour, from the Crow's Bill signifying putrefaction to the philosophical solvents disguised as Green Lions, Red Dragons, and Grey Wolves. While the process of yellowing, or citrination, often appears in medieval recipes, it seems to have interested commentators less than the more familiar processes of blackening, whitening, or reddening. Yet beyond these canonical colours, yellowness turns out to be ubiquitous in alchemy and its associated craft practices, both in Latin texts and vernacular translations. This paper uses source criticism and experimental reconstruction to interrogate the role of yellowness at the beginning, middle, and end of practice, focusing on fifteenth- and sixteenth-century England. As starting ingredients, yellow vitriol and litharge offered the potential for transmutation but also posed problems for identification and preparation. As an intermediate stage, yellowness offered promising signs of future success, in the form of dramatic colour changes and unexpected products. But yellowness also offered an end in itself, as appears from the many citrination processes attested in recipe collections which aimed to imitate the properties of gold – suggesting that yellowing was prized as a significant indicator of chemical change across diverse areas of craft practice. This paper critically examines the White Paper, Opportunity for all, published by the UK Government's Department for Education (DfE) in March 2022. This has a number of recommendations for schools in an attempt to 'level up'. In particular, there is a promise to deliver 'a fully trust-led system with a single regulatory approach [and] a clear role for every part of the school system'. Such a system provides a serious challenge to the way that Religious Education (RE) structures in England are currently built: in short, when Local Authorities no longer have schools under their control – what is the point of a SACRE? Arguing that the 'local settlement' for RE serves two purposes – a curricular purpose and support and monitoring purpose – this paper will suggest that future RE curricula will be planned at the Trust level, with the monitoring and support functions being moved from the local to the regional. Background and objectives: Pulp bleaching is a concern of many conservationists. Research has been conducted with the aim of evaluating some traditional bleaching materials used in the preservation of documents and books, such as hydrogen peroxide and sodium borohydride, which shows that these materials do not significantly improve the mechanical properties of historical papers. Also, investigating the effect of reducing agents on oxidized papers has shown the effectiveness of tert-butylamine borane complex. In another study, five dyeing processes with oxidizers (calcium hypochlorite, hydrogen peroxide in two concentrations, light bleaching and potassium permanganate) and two dyeing processes with reducing agents (sodium borohydride and tert-butylamine borane) were compared. Their effect on alfa cellulose (Whatman filter paper) and two different historical papers have been investigated. The results indicate that all the tested dye removal methods have led to a clear increase. Due to the differences of opinion, in this research, the effectiveness of two reducing agent sodium borohydride and sodium dithionite on the appearance and structural characteristics of paper is investigated. Methodology: In this research, Whatman Filter Paper No. 1 (Whatman Filter Paper No. 1) made in England with a thickness of 180 microns, diameter of 11 cm, basis weight of 87 grams per square meter and ash content of 0.06% was used. . Chemical reagents including potassium persulfate, sodium borohydride and sodium dithionite reagent grade were prepared in a laboratory from Merck, Germany.Three solutions including potassium persulfate (2% by weight/volume), sodium borohydride (1% by weight/volume), sodium dithionite (1% by weight/volume) in distilled water (distilled water, pH; 7), to perform tests on zinc. Paper samples have been used by immersion. Whatman filter paper samples were subjected to oxidation in 2% peta persulfate in distilled water for 2 days (48 hours) and then the samples were immersed. Next, the identified samples were modified by two solutions of sodium borohydride and sodium dithionite 1% in distilled water for 1 to 5 hours and then purified in distilled water. Also, the samples were subjected to accelerated aging according to ASTM standard number D4714-96 at a temperature of 90±2 centigrade and a relative humidity of 50±2% for 384 hours. Test methods include potentiostat, colorimetry, tensile strength, pH measurement, scanning electron microscope and infrared examination. Results: The results show that potassium persulfate is the most neutralizing solution among the three solutions used on the first day, and it also indicates the reducing power of two solutions of sodium dithionite and sodium borohydride, which according to the test results, sodium borohydride is the reducing power compared to sodium dithionite. The sample prepared with pH 3.92, after accelerated aging, was accompanied by an increase of several units of pH after modification with two reducing substances. Examining the data obtained from the tensile strength test shows that in comparison with the tested samples with a tensile index of 1.06, the samples modified with sodium borohydride and sodium dithionite increased the index in all five modified periods. The infrared spectrometry investigation of the samples modified with sodium borohydride and sodium dithione shows that this old material improves the structure of cellulose after aging by changing the C=O bonds to C-O and reducing the area of 1640 cm-1.. The reduction of the absorption band in the region of 1640 cm-1 has been opened in almost all time cases with this material compared to the modified sample. Based on the data, samples modified with sodium borohydride and sodium dithionite had a significant increase (approximately 25 units) in the Lfactor (light-darkness) after aging, indicating that two substances have increased the brightness of the sample by reviving the color compounds. In the microscopic examination of the surface of the fibers, in the samples identified with potassium persulfate, local separation of the fibers is observed. In the samples modified with sodium borohydride and sodium dithionite, fibers with higher consistency and entanglement are observed. Conclusion: Double bonds and carbon groups in paper samples formed with potassium persulfate have been the main responsible for the yellowness and darkness of the paper. The reducing agents, sodium borohydride and sodium dithionite reduce this aldehyde and ketone to hydrocarbon. With the loss of multiple conjugates, the light absorption of the paper is reduced and this causes the paper to brighten. The double bonds and their regeneration also cause rearrangement of the cellulose chain, which has resulted in increasing the mechanical resistance and strength of the paper samples. Background Mothers who have children removed from their care often have complex needs. These women have poor health outcomes and are dying earlier than their peers from preventable and amenable causes. Yet there is little known about how health care services might mitigate these risks. This study aimed to listen to the voices of women who had children removed from their care to understand their experiences of health and healthcare. Methods We used a narrative approach to collect and analyse interview data with six mothers who had experienced child removal in England. Each participant was asked to reflect on their life and main health challenges. Results Three narrative subplots were developed to consolidate experiences of unmet health need: (i) 'on paper you're normal': narratives of complex need, (ii) 'in my family, everyone had issues': narratives of whole family need and (iii) 'I'm still mummy, no matter where they are': narratives of maternal identity and health. Conclusions Findings highlight limitations within current systems of support, including a culture of distrust and women falling between the gaps of services. Purpose: To report distribution of codes associated with a rheumatoid arthritis (RA) diagnosis recorded in Clinical Practice Research Datalink (CPRD) Aurum compared to the previously validated CPRD GOLD database as a critical step toward making decisions about CPRD Aurum's suitability for medical research.Patients and Methods: We analyzed the distribution of codes for RA diagnoses, labs, and treatments in the new CPRD Aurum database, compared to the CPRD GOLD database by selecting relevant indicators of RA diagnosis, treatment, and clinical care. We included all patients in England in CPRD Aurum and CPRD GOLD with an incident diagnosis code for RA on or after 1 January 2005 and at least two years recorded data before first RA diagnosis.Results: We found 53,083 and 18,167 patients with a new diagnosis code for RA in CPRD Aurum and CPRD GOLD, respectively. In both databases approximately 67% were female with similar mean ages at first diagnosis. There were few differences in RA-related recording patterns between the two data sources. Before first RA diagnosis, CPRD Aurum patients had more RA-specific labs and other supporting clinical codes. After diagnosis, CPRD Aurum patients had more RA diagnoses coded and more often had 10+ general RA labs than patients in CPRD GOLD. More CPRD GOLD patients had 10+ prescriptions for conventional disease-modifying antirheumatic drugs (cDMARD) compared to CPRD Aurum. Otherwise, the distribution of drugs used to treat RA was similar between databases. The standardized incidence of RA was similar between databases.Conclusion: Overall, among patients with a diagnosis code for RA, recording of diagnoses, prescription drugs, and labs were similar between CPRD Aurum and CPRD GOLD. Slight differences were found for a few variables, but overall, we found consistency between the databases. In addition, standardized incidence of RA was similar between databases. This is the great merit of Da Rold's book: it is a truly interdisciplinary study of paper in medieval England. Da Rold argues that paper had affordances that parchment simply did not have. This paper builds on a previous article describing an innovative approach to enhance the service user and practitioner's experience of novel collaborative approach to service improvement. It aims to explore the impact of this through the voices of service users as collaborators and co-authors. The Recovery and Collaborative Care Planning Café (RCCPC) designed with World Café principles, created a safe space to foster inquiry and learning about recovery between service users, carers, and practitioners in an NHS Trust. An important part of the method was in changing conversations towards recovery and living well with conditions applying CHIME concepts (Connectedness, Hope, Identity, Meaning, and Empowerment). Story is a qualitative method known as a well-established effective means of engaging others in sharing experiences and perspectives. Through the stories of service users' experiences participating in the RCCP Cafe, it was clear that they felt that CHIME concepts had a positive impact on the way that their conversations evolved. Purpose: This paper aims to outline the initial pilot use of the client complexity matrix (CCM), an outcome measure developed for an NHS Adult Intellectual Disabilities Intensive Support Team (IST) in the West Midlands of England. Design/methodology/approach: A mixed method approach was used to report the results and evaluate the use of the CCM within this IST. The CCM was trialled for six months between January 2022 and June 2022. CCM scores pre and post IST intervention were analysed via t-test, and qualitative feedback from clinicians is detailed. Findings: Analysis suggests the CCM captures service success showing a significant decrease in complexity and risk post-intervention. Qualitative comments from clinicians were generally positive; however, there were requested changes regarding specific presentations, use of labels/language and for a digital version. Research limitations/implications: The tool is not co-constructed with the client. Because of this IST's caseload, the sample size was small. Only one clinician was asked to complete the CCM pre and post intervention. Future use of the measure will encourage two clinicians to complete the measure to evidence its inter-rater reliability. Each client's specific presentation was not explicitly analysed here. Practical implications: The CCM will be updated using the feedback from clinicians and from the peer review process. Research that examines the generalisability of this measure to other IST services would be beneficial. School-based education in England has undergone significant changes since 2010, with a huge expansion of academies, schools outside local authority control, funded directly by central government. Academies and local authority (LA) maintained schools are subject to different legislative and regulatory frameworks. This paper focuses on the governance of LA maintained schools, single academy trusts (SATs) and schools that are part of multi-academy trusts (MATs). The research involved analysing legislative provision, policy documents, and documents addressing the governance arrangements of a sample of 23 secondary schools. Our findings reveal a fragmented state-funded secondary school system as regards overall governance, school admissions, the curriculum, and the use of funding. Significantly schools in MATs, which are governed by the trust board, lack the autonomy of either SATs or maintained schools and are instead under the ultimate control of the trust board. Examining the entanglement of democracy and social justice in education and the relationship to social mobility, this paper critiques the individualising nature of social mobility in policy discourse as inimical to human flourishing and education as a public good. The rhetoric of social mobility which responsibilises individuals for their success, without due regard to the systemic changes needed to enable this and the societal barriers to social mobility experienced disproportionately by some groups in society, is detrimental to social justice. A conception of education as a pathway to social mobility must be located in a wider vision of civic engagement in a more inclusive and fairer society. Yet civic engagement in education at the local level has been eroded, as illustrated in this paper by the example of an evolving model of school governance in England. As a corporatist, market driven reform, this model exemplifies how local democratic ties between schools and engagement with their communities can be undermined. Social mobility to enable opportunities, access and participation in democratic civic society becomes a fantasy when society is riven with systemic inequalities, lacks the necessary conditions to enable human flourishing and links to community engagement in education for democratic renewal are downplayed. This paper reviews the research on successful school principalship carried out in England over the last 20 years. Drawing on evidence synthesized from the International Successful School Principalship Project (ISSPP) and related English school leadership research conducted by ISSPP scholars, this review aims to answer a conceptual research question: How do the principalship's moral purposes and contextual understanding shape the time-sensitive, informed adoption of combinations and accumulations of strategies that lead to sustained school success? This paper identifies five research insights derived from case studies in England and elaborates on the complex, values-led layered web of interactions between the school principal and key stakeholders within and outside the school in the context of frequent social changes and policy interventions in England. Whilst the pace has been greater and more intense than in many other countries, the direction has been, and remains, similar. The body of scholarship here reviewed engages with national policies as filtered and then enacted by successful principals. While 'effective' principals lead to students' success as measured by performance on national tests and examinations, our focus is upon an empirically founded definition of 'successful' school leadership that is located in complexity theory and encompasses the enactment of the core purposes of education that include but go beyond the functional. In doing so, it avoids 'what to do' formulae and the limitations of certain theoretical 'leadership' models, finding that successful school leaders' work embodies a broader humanistic view of student learning and achievement, which implies the preordinance of the personal over the functional. Taken together, these research insights contribute to the ISSPP's continued effort to refine and advance the knowledge base of successful school leadership within and across different countries. This contribution examines Sismondi's money, banking and credit theories and explores his public debt analysis (1803–1838) to connect the instability of market economy with his vision of the social contract. A detailed analysis is offered of the evolution in Sismondi's opinion on the nature of money and the banking system, and the part it plays in his trade cycle theory. Sismondi's monetary thought is then contextualised with a discussion of his policy-mix in relation to the Napoleonic war financing in Continental Europe. Connections with the upcoming flood of literature in England on the bullion controversy are also offered. Remarks are then suggested on the progressive emergence of an "art of public borrowing" according to which the people who provide the money also control the government. Finally, some reflections are proposed on the explicit connection established by Sismondi between budget deficits, the (ab-)use of inconvertible paper money and the partial collapse of the social contract initiated by banks and the governments using it. The entrenched instability of a market economy (discussed in an earlier article) is reinforced by the banking/credit system that works along similar line than any wealth-producing firm. Objective: This work aims to analyze the 100 most cited papers in radiotherapy or chemoradiotherapy for cervical cancer. Methods: The 100 most cited papers in radiotherapy or chemoradiotherapy between 1990 and 2020 were identified with Thompson Reuters Web of Science citation indexing on August 24, 2020. The publication years, source titles, countries/regions, total citations, and average citations per year were extracted from the Web of Science. The research type and research domain were classified by the authors. Results: These 100 papers were cited a total of 28,714 times, and the median number of citations was 169.5 (ranging from 116 to 1,700 times). The most cited paper was "Concurrent cisplatin-based radiotherapy and chemotherapy for locally advanced cervical cancer" by Rose et al., with a total citation of 1,700 times. The International Journal of Radiation Oncology Biology Physics (40 papers), Radiotherapy and Oncology (14 papers), and the Journal of Clinical Oncology (12 papers) published the largest number of papers. USA (47 papers), Austria (18 papers), Canada (13 papers), and England (13 papers) contributed the largest number of papers. Image-guided adaptive brachytherapy (IGABT) had the largest number of papers (25 papers). Concurrent chemotherapy was the most cited research domain, with 10,663 total citations and 592.4 citations per paper. Conclusion: In the present study, we conducted a bibliometric analysis of the 100 most cited papers in radiotherapy or concurrent chemoradiotherapy for cervical cancer in the past 30 years. The last 30 years has seen an exponential increase in Historical Institutional Abuse Inquiries.1 One feature of these has been to place adult survivor voices at the center of Inquiry work, meaning that child abuse victims and survivors2 are engaging with Inquiries, sharing their experiences, with this participation often presented as empowering and healing. This initiative challenges long held beliefs that child sexual abuse survivors are unreliable witnesses, which has led to epistemic injustice and a hermeneutical lacunae in survivor testimony. However to date there has been limited research on what survivors say about their experiences of participation. The Truth Project was one area of work of the Independent Inquiry into Child Sexual Abuse3 in England and Wales. It invited survivors of Child Sexual Abuse to share their experiences including the impacts of abuse and their recommendations for change. The Truth Project concluded in 2021 and heard from more than 6,000 victims of child sexual abuse. The evaluation of the Trauma Informed Approach designed to support survivors through their engagement with the project was a mixed methods, two phase methodology. A total of 66 survey responses were received. Follow-up interviews were conducted with seven survey respondents. The Trauma Informed Approach was found to be predominantly helpful in attending to victim needs and minimizing harm. However, a small number of participants reported harmful effects post-session. The positive impacts reported about taking part in the Truth Project as a one-off engagement challenges beliefs that survivors of child sexual abuse cannot safely talk about their experiences. It also provides evidence of the central role survivors should have in designing services for trauma victims. This study contributes to the epistemic justice literature which emphasizes the central role of relational ethics in the politics of knowing, and the importance of developing a testimonial sensibility when listening to marginalized groups. This paper explores Digital Poverty (DP) in England by adopting the DP Alliance's theoretical framework that includes both Individual Determinants (individual capability and motivation) and Circumstantial Determinants (conditions of action). Such a framework is interpreted as an expression of Strong Structuration Theory (SST), by situating the connection between social structure and human agency in an intertwined relationship. We focus on new potential vulnerabilities that are connected to DP in England by drawing on a survey conducted on a randomised stratified sample (n = 1988) of parents aged between 20–55 with children at school. Exploring parents' experience in the COVID-19 era, we identified economic factors and having children with disabilities as important predictors connected to Digital Poverty. Additional socio-demographic traits (such as age and education), parental status, lifestyles and digital behaviours also play a role in predicting some of the determinants linked to Digital Poverty. In an era of accelerating biomedical innovations, partial ectogenesis looms as a double-edged sword for female bodily autonomy and reproductive rights. Rather than simplifying or 'solving' abortion access and the ethical issues inherent to that right, this technology wields the alarming potential to curtail women's right to safe and legal abortions. This article investigates the potential of partial ectogenesis as a 'solution' to the abortion debate, focusing on two significant hypotheses. Foremost, the paper scrutinizes the dilemma of ectogenesis abortion; specifically, instances where a woman intending to terminate her pregnancy is obliged to undergo foetal transfer surgery. This scrutiny engages with Räsänen's concept of the right to the death of a foetus, yet ultimately concludes that this supposed right is unconvincing. Despite there being no such right, this paper argues that the prospect of partial ectogenesis is incapable of resolving the core issues of the abortion debate, not least because of the inherent conflict between a woman's right to her own body and the proposed technology or rather, compulsion of such surgeries. Instead, this article posits that a more significant ethical and logical concern lies in misconceptions surrounding the term 'foetus' and how such misconceptions impact considerations of autonomy. Thereafter, this paper examines the potential for partial ectogenesis to alter the legal viability threshold set by the Abortion Act 1967-in particular, this paper explores the impact a shift in the viability threshold would have on female bodily autonomy. In the aim of resolving these concerns, this article concludes by advocating for the decriminalisation of abortion to safeguard female bodily autonomy against constraints on abortion access were the viability threshold legally revised. By addressing these crucial topics, this article aims to provide a nuanced analysis of the dialogue surrounding reproductive rights, bodily autonomy, and the complex interplay between emerging biotechnologies, medical ethics and legislative frameworks. The paper discusses the role of private planning consultants as intermediary-actors and their implications in relation to planning theory and practice. To do so, the paper focuses on niche consultants involved in servicing neighbourhood scale plan-making in England, clarifying that they hold crucial agency in local planning processes and adding to the understanding of consultancy roles and co-production dynamics in planning. The paper draws together the literature on private sector consultancy and on intermediaries, along with theoretical work highlighting the diversification of planning, the rise of the collaborative turn and the effect of regulation on creating niche markets in planning expertise. The conclusions drawn help clarify the 'action on others' that planning consultants, as intermediary-actors, have in collaborative governance and planning in and beyond neighbourhood planning. Purpose: There has been limited research on why football clubs contribute to charity. This paper examines how football clubs and their charitable conduits report information when discussing their connectedness. In addition, it explores reasons why, and the extent to which, football clubs support altruism via such charitable vehicles. Design/methodology/approach: Case studies of four major football teams (Manchester City/Manchester United in England and AC Milan/Inter Milan in Italy) are discussed, with formal reports of the clubs and their associated charitable conduits being analysed. Findings: Boundaries between the clubs and their charitable conduits are frequently blurred. Evidence suggests that acknowledging the co-existence of different factors may help to understand what is reported by these organisations and address some of the caveats in terms of autonomy and probity of their activities and reporting practices. Research limitations/implications: The research uses case studies of four major 'powerhouses' of the game and their associated charitable spinoffs. While this is innovative and novel, expanding the research to investigate more clubs and their charitable endeavours would allow greater generalisations. Practical implications: The study provides material that can be used to reflect on the very topical subject of 'sportswashing'. This has the potential to input to deliberations relating to the future governance of the game. Originality/value: The paper explores relationships between businesses and charities/nonprofits in a sector so far little investigated from a charitable accountability perspective. It suggests that motives for engaging in charitable activity and highlighting such engagement may extend beyond normal altruism or warm-glow emotions. [ABSTRACT FROM AUTHOR]

An increasing body of research suggests that bilingualism is possible and perhaps even advantageous for autistic individuals. However, several factors might influence parents' decisions about raising their autistic child bilingually, including national language policies, educational contexts, advice received from key professionals, and the child's individual strengths and needs. Accordingly, there is a clear imperative to understand how the views of different stakeholders converge and diverge when language decisions are made in the context of autism. This paper brings new insights by synthesising the findings of three qualitative studies that used interpretative phenomenological analysis (IPA) to explore the perspectives and experiences of bilingual autistic children (n = 11), parents (n = 16), and educational practitioners (n = 13) of bilingualism in autism in England and Wales. Despite wide variation between and within groups, a striking tension emerged between individuals' beliefs about bilingualism in general, which were positive, and their experiences of bilingualism in autism specifically, which often foregrounded more monolingual approaches. This paper examines this tension, with a particular focus on stakeholders' attitudes towards the feasibility of bilingualism, families' language choices in the context of autism, and how notions of contextual linguistic diversity accentuated differences between England and Wales. We conclude by arguing that greater awareness of both bilingualism and neurodiversity in educational and clinical settings may improve the experiences of bilingual autistic children and, crucially, the language advice families receive. Introduction: Associations between structural inequalities and health are well established. However, there is limited work examining this link in relation to mental health, or that centres public perspectives. This study explores people's experience and sense‐making of inequality in their daily lives, with particular consideration of impacts on mental health. Methods: We conducted a peer research study. Participants had to live in one of two London Boroughs and have an interest in inequalities and mental health. Using social media, newsletters, local organisations and our peer researchers' contacts, we recruited 30 participants who took photos representing their experience of inequality and discussed them during semi‐structured interviews. Data were analysed using reflexive thematic analysis. Results: Three themes were identified in this study: (1) inequalities are unjust, multilayered and intertwined with mental health. Accounts demonstrated a deep understanding of inequalities and their link to mental health outcomes, describing inequalities as 'suffering' and 'not good for anyone'. Financial, housing, immigration and healthcare problems exacerbated poor mental health, with racism, gender‐based violence and job loss also contributing factors for both poor mental health and experiences of inequality; (2) inequalities exclude and have far‐reaching mental health consequences, impacting personal sense of belonging and perceived societal value and (3) moving forwards—addressing long‐standing inequality and poor public mental health necessitated coping and resilience strategies that are often unacknowledged and undervalued by support systems. Conclusion: Lived experience expertise was central in this study, creating an innovative methodological approach. To improve public mental health, we must address the everyday, painful structural inequalities experienced by many as commonplace and unfair. New policies and strategies must be found that involve communities, redistributing resources and power, building on a collective knowledge base, to coproduce actions combatting inequalities and improving population mental health. Motivated by demand prediction for the custodial prison population in England and Wales, this paper describes an approach to the study of service systems using infinite server queues, where the system has non-empty initial state and the elapsed time of individuals initially present is not known. By separating the population into initial content and new arrivals, we can apply several techniques either separately or jointly to those sub-populations, to enable both short-term queue length predictions and longer-term considerations such as managing congestion and analysing the impact of potential interventions. The focus in the paper is the transient behaviour of the M t / G / ∞ queue with a non-homogeneous Poisson arrival process and our analysis considers various possible simplifications, including approximation. A growing body of scholarship draws attention to prisons and environmental justice, pointing out the propensity for prisons to be located on contaminated sites and to be in close proximity to polluting industries, as well as for prisons themselves to contribute to local environmental degradation. Prisoners' immobility renders them unable to relocate away from harmful environments, and there are now numerous suggestions that their wellbeing suffers as a result of the poor quality of many local environments. However, since a relationship between environmental quality and prisoner wellbeing is yet to be robustly demonstrated, there is currently no firm evidence base from which to argue for positive change. This paper therefore examines the effect of the environmental quality of the locations of prisons, approximated as the presence of greenspace in the immediate vicinity of prisons, local species biodiversity and local levels of air and noise pollution, on wellbeing outcomes in a set of prisons in England and Wales. It finds that good environmental quality, in the form of high biodiversity and/or low air pollution, enhances the already-recognized positive effects of greenspace on the wellbeing of incarcerated populations. This paper presents a systematic literature review exploring the Family Group Conferencing experiences with racially minoritised families in England. The literature search identified eight qualitative studies that met the inclusion criteria. This review found that racially minoritised families felt that Family Group Conferencing was empowering and met their cultural needs at times, but suggested caution about how culturally adaptable the approach is for this population. We argue that FGC could be presented as an Indigenous model of engaging with family networks with relevance for social workers working with racially minoritised communities in England. We suggest that Family Group Conferencing aligns with aims of decolonisation and cultural competence and argue the need to explore how Indigenous knowledge can be valued more effectively in global social work debates. This article explores the impact of the 'hostile environment' on racialised migrant women's experiences of pregnancy and childbirth in England, arguing that the 'hostile environment' functions as a technology of 'stratified reproduction.' First coined by Shellee Colen, the concept of stratified reproduction describes the dynamic by which some individuals and groups may be supported in their reproductive activities, while others are disempowered and discouraged. This paper locates the stratified reproduction produced by the 'hostile environment' as intertwined with wider gendered and racialised discourses around British citizenship which have been 'designed to fail' racialised residents of the UK. Drawing on interviews with racialised migrant mothers in the north of England, this paper analyses how the proliferation and intensification of immigration controls interacts with gender, race, class, and other social regimes to differentially allocate the resources necessary for a safe and healthy pregnancy and childbirth, and how this is experienced materially by pregnant migrants. Background: Several cross-sectional studies have investigated the incidence of urinary Congo-red dye positivity in women with preeclampsia (PE), compared to unaffected pregnancies, and reported very high sensitivity and low false positive rate in the diagnosis of PE. Objective: To determine the performance of the urinary Congo-red dot paper test at 35–37 weeks’ gestation in the prediction of delivery with PE at ≤2 and >2 weeks after assessment. Methods: This was a prospective observational study in women attending for a routine hospital visit at 35+0 to 36+6 weeks’ gestation in a maternity hospital in England. Urine samples were collected and the Congo-red dot paper test was used to assess the degree of Congo-red dye positivity. The test uses a scoring system from 1 to 8 and the higher the score the greater the degree of Congo-red dye positivity. We examined and compared the degree of Congo-red dye positivity in the groups that delivered with PE at ≤2 and >2 weeks with those that remained normotensive. Reproducibility was assessed by examining the inter- and intra-observer reliability of scoring on stored images with the researchers blinded to previous results. Results: The study population of 2140 women included 46 (2.1%) that subsequently developed PE (2.1%). The urinary Congo-red dot test was positive in 8.3% (1/12) and 2.9% (1/34) that delivered with PE at ≤2 and >2 weeks from assessment and in 0.2% (4/2094) of the unaffected pregnancies when the cutoff for Congo-red dye positivity was ≥5. The respective values when the cutoff used was ≥3 were 66.7%, 23.5%, and 16.5%, respectively. The intraclass correlation coefficient for the inter-observer reliability was 0.926 (95% CI 0.890–0.953, p<.0001) and Cohen's kappa coefficient for the intra-observer reliability was 0.904, p<.0001. Background: The National Health Service (NHS) and general practice are increasingly adopting digital services. These services can impact both positively and negatively upon patient experiences, and access to digital services is not equal amongst all groups. Within a wider project examining digital facilitation (the Di-Facto study) our team conducted a patient survey amongst English primary care practices aiming to investigate patient views of what supports uptake and use of web-based services. This paper reports on the analysis of the free-text responses from the patient survey. Methods: The Di-Facto patient survey was distributed to practices in eight clinical commissioning groups (CCGs) in England between 2021–2022. We examined free-text responses to two questions relating to access to primary care web-based and support for web-based services. We used qualitative reflexive thematic analysis based on a six-stage process to analyse responses. Results: Of the 3051 patients who responded to the Di-Facto survey, 2246 provided a free-text response. We present our findings in two major themes: systems and structures and their impact on use of web-based services, and 'what works for me', a description of how respondents described what worked, or did not work in terms of their interactions with web-based services. Respondents described how the technology, such as poor practice website design, confusion over multiple digital apps, data security and concerns about eConsultation offerings impacted on use of web-based services. Respondents described practice level barriers, such as a lack of or inconsistent provision, which prevented optimal use of web-based services. Respondents described personal and technical barriers that impacted on their use of digital services, and described which web-based services worked well for them. Respondents felt that web-based services were not a replacement for face-to-face interactions with a doctor. Conclusions: This analysis of free-text responses from a large patient survey highlights the system, practice, and person level barriers and facilitators to use of digital services in primary care. With an increasing push towards digital solutions in NHS primary care, practices should consider the design, rollout and communication of their web-based services to support patient access. Policies for biodiversity no net loss and net gain underwrite narratives for green growth through advancing reparative logics to ongoing habitat impacts. By enabling offsetting practices that risk accommodating rather than averting land change developments, net principles are said to resemble modes of 'accumulation by environmental restoration'. Biodiversity net principles are frequently depicted visually as a diagram of the mitigation hierarchy for communicational ease and have proliferated over recent decades despite little evidence for their ecological effectiveness. This paper combines economic sociology, visual media analysis of the net diagram and political ecology to account for the stabilisation of net principles in policy frameworks. It highlights the upstream imaginative work that this visual tool and its wider assemblages perform to support offsetting and habitat banking practices on the ground. The paper positions the NNL diagram as a conceptual and ideational technology. It traces the practices through which biodiversity is rationalised by the Cartesian coordinates of an XY schematic, and en-framed as a measure of numerical value on a vertical scale. The effect is to engender coherence to the idea of netting out differences in aggregate sums of biodiversity unit value, making nature conceptually offset-able. I develop this account through a history of the diagram as well as the broader processes that have shaped the policy and its arrival in English planning frameworks. Observers increasingly question how biodiversity offsetting and no net loss/ net gain have become so popular when their empirical foundations are so weak. This paper proposes that within the wider assemblages of actors, one answer is located in the potency and mobility of conceptual technologies such as diagrams of no net loss or net gain of biodiversity and the logic of balance-sheet accounting that is imbricated within the visual design. This paper reports the study which explored the learning and teaching of the topics of climate change, sustainability and disaster risk reduction in secondary-level teacher education programs in England and Japan. Through interviewing teacher educators, the study particularly probed how teacher education programs used local knowledge and collaboration in discussing the above topics. Geography tends to be the main subject area for these global agendas, but its crammed curriculum is "an ongoing challenge" for teacher education. Some researchers demonstrate that university-based initial teacher education has "ignored" training teachers on how to implement environmental and sustainability education at schools. Besides, the inquiries into how initial teacher education equips geography teachers for the learning and teaching of disaster risk reduction are still scarce. Textbooks are by no means ideological. This means that they present their dominant values and culture in the foreign-language education. In other words; textbooks can change thoughts of learners of the target language. The study is limited to the Iraqi ESL textbooks (English for Iraq) for sixth preparatory stage. This paper tries to examine the discourse of these textbooks critically. The aim of the current paper is to investigate the national identity of the publishing house in the Iraqi textbooks. In order to reveal the national identity embedded in them, the study draws on a corpus-based analysis. This study found the frequency of referring to nation identity of England more than other countries throughout the whole books. Early-career crafts graduates often face a perceived dilemma, that of balancing passionate work with their need to make a living. This paper explores the negotiation undertaken by early-career crafts graduates and the entrepreneurial strategies they adopted which variably combine the logics of passion and economics. The paper draws on qualitative interviews with 25 early career crafts graduates in England conducted in 2018. Five strategies -support, streams, synthesis, segment and synergy – are identified, including the different income generation and creative production models they represent. The challenges associated with particular strategies are also considered. The identification of these strategies provides a more nuanced understanding of the organisation and management of craft practices, particularly during the early career period, that goes beyond the acknowledgement of portfolio working as prominent feature of creative work. The paper contributes to the literatures on the organisation of craft work and creative entrepreneurship by identifying ways in which craft practitioners structure and manage their work to sustain and develop their practice. It argues that the positioning of craft micro enterprises as hybrid organisational forms provides an opportunity to redress the binary opposition between passion and work in the creative economy in favour of a more balanced approach. A call for further research on craft production at the micro-enterprise and sole trader level is made to develop new knowledge and recommendations to support the growth and sustainability of the craft sector. Purpose: This paper aims to explore the links between being lonely and isolated, and increased risks of abuse for adults with care and support needs. Design/methodology/approach: Thematic analysis was used to explore features of loneliness and social isolation present in South Yorkshire Safeguarding Adults Reviews (SARs) published since 2014. Findings: Ten out of fifteen SARs indicated there had been issues of loneliness and/or social isolation for the person who was the subject of the SAR. Research limitations/implications: The limitations of this paper are that it only included SARs from the South Yorkshire area. Future research should explore national and international perspectives on these issues. Practical implications: Safeguarding Boards should include actions to address loneliness and social isolation as part of prevention strategies and services to develop approaches that can minimise or prevent abuse before it occurs. Practitioners should routinely explore whether the people they work with feel lonely and/or isolated and support people to take appropriate action to mitigate these risks. Current research on Digital Twin (DT) is largely focused on the performance of built assets in their operational phases as well as on urban environment. However, Digital Twin has not been given enough attention to construction phases, for which this paper proposes a Digital Twin framework for the construction phase, develops a DT prototype and tests it for the use case of measuring the productivity and monitoring of earthwork operation. The DT framework and its prototype are underpinned by the principles of versatility, scalability, usability and automation to enable the DT to fulfil the requirements of large-sized earthwork projects and the dynamic nature of their operation. Cloud computing and dashboard visualisation were deployed to enable automated and repeatable data pipelines and data analytics at scale and to provide insights in near-real time. The testing of the DT prototype in a motorway project in the Northeast of England successfully demonstrated its ability to produce key insights by using the following approaches: (i) To predict equipment utilisation ratios and productivities; (ii) To detect the percentage of time spent on different tasks (i.e., loading, hauling, dumping, returning or idling), the distance travelled by equipment over time and the speed distribution; and (iii) To visualise certain earthwork operations. Purpose: The aim of this paper is to respond to a gap in the literature around resilience in later life for older people from minority groups of identity or experience. Specifically, it argues that we need to pay greater attention to how access to different types of social capital may leave some older people more or less able to cope with adverse events and how experiences of social exclusion can limit access to important networks of support during times of crisis. Design/methodology/approach: This paper draws on research conducted to inform the resilience strategy of Greater Manchester in 2019, and specifically looked at how this strategy could better address the needs of the regions' diverse ageing population. It used a qualitative design including focus groups with older women of Punjabi heritage living in Greater Manchester, interviews with staff from a community and voluntary organisation working with these women, and interviews with staff at an organisation supporting refugees and asylum seekers in Greater Manchester who specifically worked with older adults. Findings: The research found that belonging to a minority group and experiences of social exclusion gave participants in this study both resources and vulnerabilities when it came to dealing with external shocks in later life. Whilst participants in this study had access to strong networks of bonding capital based on shared identity and experience, social exclusion often meant they faced barriers to accessing network of support outside of these communities. Research limitations/implications: Findings from this study have implications for both future research and policy. In the case of the latter there are implications for those working in resilience planning in terms of how to address the needs of diverse older populations. For researchers, this paper has implications for how we understand the impacts on inequality in later life particularly in the context of the COVID-19 pandemic. Originality/value: The originality of this paper lies in its consideration of the impact of inequalities and social exclusion on the resilience of older people in times of crisis. Livestock markets are pathological sites in which contrasting biopolitical regimes compete to reconfigure agricultural practices and identities. Whilst the circulation of cattle is central to agricultural geographies, little is known about the practices of cattle trading or the role of livestock markets in cattle purchasing. Drawing on recent attempts to conceptualise the process of marketisation, this paper seeks to invigorate research into livestock markets. Specifically, the paper conceptualises cattle purchasing as a market encounter in which socio-technical arrangements, devices and bodily performances entangle cattle and farmers, enabling markets to work. Using data collected from interviews, focus groups and participant observation at livestock markets in England, the paper makes two contributions. Firstly, the paper shows how farmers' cattle purchasing practices are organised by practices of 'fluid engineering' that seek to maintain the 'farm system'. Secondly, the paper shows how at livestock markets, these strategies are mediated by front and backstage 'market displays' by farmers and auctioneers which produce market price through a series of performances that are carefully spatially and temporally ordered. Specifically, these displays perform specific rural and agricultural identities, such as the 'genuine' or 'good' farmer. In creating these spatial frames, frontstage displays diminish the relevance of backstage displays that rely on abstract calculations by distant others. The paper therefore reveals the intense entanglements and socio-technical work that is required to make cattle markets function and their wider relevance for the management of livestock diseases. Human geographers have analyzed the co-constitutive relationship between race, gender and sexualities across different spaces and social contexts and have called for intersectional approaches in discussions of identities, power and space. This article applies an intersectional framework to the processes of sexualization, racialization and exoticization that shape the daily lives and erotic/romantic encounters experienced and narrated by participants to two different projects: gay and bisexual men from a North African background living in Belgium; Italian gay men living in England; non-White gay men living in Italy. By discussing qualitative data collected during interviews with these men, and through a continued dialogue about this data between the authors, the paper explores both the effects of these processes on the lives of participants, and the strategies they enact to navigate their social worlds. The focus is on two elements, central to participants' narratives: the specificity of the intersectional experience of encountering men who expect a specifically gendered and racialized performance based on 'roughness' and 'wildness', and the capitalization on these exoticizing and racializing images to increase one's desirability on the dating/hook-up scene and everyday social and work life. By highlighting these elements, this paper shows the importance of applying an intersectional approach to analyses of the entanglements of racialization and sexualization in order to complicate linear accounts of these processes. Increasing rates of gender-based and sexual abuse, coupled with a rise in misogynistic influencers online, have become a growing issue in UK and Irish schools. This paper reports on the findings of a postlockdown study in England and Ireland that piloted workshops on gender-based and sexual violence. While most student responses were positive, we found that roughly 10% of girls and 20% of boys were resistant. In this paper, we explore these critical responses, focusing specifically on male resistance. Our findings indicate that new strategies, which avoid the concept of 'toxic masculinity', are needed to help boys move from defensive to empathetic engagements. We also find that the neoliberal, meritocratic ethos of many schools has fostered a problematic framing of gender-based violence as genderneutral. We conclude that it is vital to adopt an intersectional, whole-school approach to educating about sexual violence, which acknowledges male victimhood, while also emphasizing gendered privileges. Accounting for the values placed on nature by the public is key to successful policies in reversing ongoing biodiversity declines. However, biodiversity values are rarely included in policy decisions, resulting in poorer outcomes for people and nature.Our paper addresses an important evidence gap related to the non‐availability of values for appraising large‐scale policies and investment programmes for species recovery and habitat improvement at the national level.We use a stated preference choice modelling approach to estimate household preferences and Willingness to Pay for species recovery and habitat improvement over a wide range of habitats in England.The framing of our stated preference study is crucial to the evidence we develop. Within the study, we define species recovery as incremental improvements to habitat quality and present respondents with choices between conservation policy options that improve different habitat types. We then use the response data to estimate values for habitat quality improvements, and the associated improvements to species presence and abundance. We are thus able to estimate economic benefits for 'wild species recovery' simultaneously across a wide range of habitat types.Willingness to pay values for habitat improvement was found to be highest for improvements from 'moderate' to 'full' species recovery by 2042; and for habitat types which have relatively low current extents in England, such as lowland fens.Policy Implications: biodiversity policy designers can make use of stated preference methods to guide decisions over which aspects of biodiversity targets to focus more resources on, since this enables policy to reflect public preferences, and thus engages higher public support for conservation. Drawing on a Foucauldian theorisation and an in-depth study with eight disabled student activists in England, this paper explores how persistent marginalisation and ableism in higher education has triggered a wave of activism among disabled students, who, just before the advent of the pandemic, had organised a structured movement, Disabled Students UK. We employ Foucault's ideas of the care of the self and others to discuss the formation of disabled students as activist subjects fighting discrimination in English higher education, in a moment in which the intersection between inclusive policies and austerity measures exposed the ableism rooted in academic practices. This paper considers the risks to Early School Leaving (ESL) as perceived by young people who are Early Leavers or at-risk of Early Leaving, and their educators in one region in South England. It draws on 38 interviews and focuses groups with 39 young people and 53 adults working in various roles across 11 educational settings including mainstream and specialist schooling, alternative learning provision and vocational education and training, as well as the local authority. Guided by a holistic framework to examine risks to ESL [Brown et al. (2021. "A Conceptual Framework for Researching the Risks to Early Leaving." Journal of Education and Work 24 (7–8): 723–739. )], the paper analyses findings according to five different categories of risk: personal challenges; familial circumstances; social relationships; institutional features of school/work and structural factors of policy, economic disadvantage and the educational system. The findings highlight that while educators focused upon structural factors and personal challenges, young people themselves identified social relationships and institutional features of school as the most significant influencers on ESL risk. The contribution of this paper is two-fold; firstly, in presenting a comprehensive analysis of the multiple risk factors to Early Leaving; and secondly, in reflecting on the difference in emphases between young people's perspectives of the key constituents of risk and those of the educators who support them. There has been an increasing acknowledgement in the UK of the importance of recognising frailty as a condition that leaves older people vulnerable to dramatic, sudden changes in health triggered by seemingly small events. In policy, the approach to managing frailty is often an emphasis on staying well with limited consideration to frailty as an end-of-life phase. Meanwhile, discharge from hospital continues to be complex. Overstretched acute hospitals are juxtaposed with community and social services that struggle to keep up with the demand of those being discharged and the labelling of older people as "bed blockers" at the centre of delayed discharges. This paper reports a study underpinned by constructivist grounded theory methodology, with the aim of exploring the experiences and perceptions of stakeholders. Semistructured interviews were conducted with 57 participants including patients, their informal carer(s), community hospital staff, community health professionals, and care home managers. The core category of "the drive to discharge conveyor belt" was derived from data analysis. A significant finding of this study was that of the carer, their burden, and their intrinsic role in facilitating discharge, filling in gaps in services, coordinating services, and enabling the patient to stay at home, with little consideration of their choices or autonomy. Estimating loss reserves is a crucial activity for non-life insurance companies. It involves adjusting the expected evolution of claims over different periods of active policies and their fluctuations. The chain-ladder (CL) technique is recognized as one of the most effective methods for calculating claim reserves in this context. It has become a benchmark within the insurance sector for predicting loss reserves and has been adapted to estimate variability margins. This variability has been addressed through both stochastic and possibilistic analyses. This study adopts the latter approach, proposing the use of the CL framework combined with intuitionistic fuzzy numbers (IFNs). While modeling with fuzzy numbers (FNs) introduces only epistemic uncertainty, employing IFNs allows for the representation of bipolar data regarding the feasible and infeasible values of loss reserves. In short, this paper presents an extension of the chain-ladder technique that estimates the parameters governing claim development through intuitionistic fuzzy regression, such as symmetric triangular IFNs. Violent injury varies widely across England and Wales as does the price of alcohol. While the links between alcohol consumption and violence are well established in the medical and epidemiological literature, a causal link is questionable. This paper cuts through the causative argument by reporting a link between the general price of alcohol and violence-related injury across the economic regions of England and Wales. It examines the influence of the real price of alcohol and identifies an 'April effect' that coincides with the annual uprating of alcohol prices for excise duties, on violence-related injuries recorded at Emergency Department attendance. The data are monthly frequency of violent injury rates covering the period 2005–2014 across the economic regions. The principal finding is that a one-way relationship between the real price of alcohol and violent injury is established, and tax policy can be used to reduce the incidence of violent injury and the associated health costs. Climatic changes can cause unpredictability in flood regimes that traditional flood risk management (FRM) approaches may struggle with. Therefore, flood resilience is seen as a supplementation to these approaches, putting a larger emphasis on flood acceptance and minimising consequences. An (emergent) group contributing towards flood resilience is civil society. This paper examines how civil society contributions can be furthered and guided in the Netherlands as well as exploring potential limitations in doing so. To achieve this, England is used as a good practice example due to a more developed and defined role for civil society being present here. Data were collected on both actual (England and the Netherlands) and potential (The Netherlands) civil society contributions. These were compared to identify potential avenues for Dutch civil society contributions to flood resilience that can be further investigated. The research shows that the most promising avenues are improving advocacy from citizens, improving local flood awareness and developing relationships between FRM authorities and existing citizen groups that can be harnessed and mobilised to support flood resilience. Additionally, the research also provides insights into potential limitations for transferring resilience approaches from one context to another beyond the cases discussed in this publication. Background: When parents bring their child to appointments and then adhere to agreed speech and language therapy (SLT) recommendations, there is the potential to increase the intensity of the intervention, support generalization and improve outcomes. In SLT, however, little is known about factors that may promote attendance or adherence. Studies in other clinical areas such in medicine, psychology and physiotherapy have identified risk factors for non‐attendance or non‐adherence that are multifactorial and variable dependent on, for example, population and intervention. Aims: To identify rates of non‐attendance and non‐adherence, and to identify parent or child factors associated with parent involvement in intervention for children under 5 years of age receiving SLT. Methods: Parents completed questionnaires at two time points assessing the domains of parents' beliefs (problem perceptions, self‐efficacy), personal circumstances (socio‐demographics, family functioning), treatment experience and child factors. Predictors of parent attendance and adherence were identified through multiple regression analyses. Non‐attendance rates were identified via local health records and non‐adherence ascertained using a specific parent‐reported measure within the treatment experience domain. Results: Participants (N = 199) were predominantly mothers, and were ethnically and socio‐economically diverse, speaking a wide range of languages. Their children presented with a range of speech, language communication needs (SLCN). The rate of non‐attendance was 25% and the main predictors of non‐attendance were maternal age, education level and two factors within the parent beliefs domain. This model explained 40% of the variance in attendance. The rate of non‐adherence in this cohort was 26% with parental rating of the importance of a recommendation and self‐efficacy beliefs predicting adherence; this explained 56% of the variance in adherence to SLT recommendations at home. Conclusions & Implications: Our research has provided preliminary evidence of the influence of parents' beliefs, personal circumstances and treatment experiences on their involvement in their child's therapy. Speech and language therapists should consider factors impacting attendance and adherence to treatment and explore parental perceptions of their child's SLCN before embarking on an intervention, a foundation for collaborative practice. A possible limitation of this study is that the levels of attrition in our sample led to generally high measured rates of participation, which should be considered in future studies. Future research should explore adherence in treatments with varying doses, with different types of SLCN or interventions and in different settings. WHAT THIS PAPER ADDS: What is already known on the subject: It is acknowledged that parent involvement in their child's therapy, such as attending and adhering to recommendations, is important but little is known about the rates of involvement and what factors may be associated with attendance and adherence in SLT. Qualitative research has explored parental involvement suggesting that beliefs about an intervention may be pertinent. Extensive research in other clinical areas suggest multiple and varied factors are influential and further exploration of particular populations and interventions is necessary. What this paper adds to the existing knowledge: This study identified rates of parental non‐attendance and non‐adherence in a cohort of predominantly mothers of children under the age of 5 years. It is the first study to measure parent adherence in SLT and identify factors that are associated parental adherence to SLT recommendations. It adds to the small body of SLT specific research in understanding risk factors for non‐attendance. What are the potential or actual clinical implications of this work?: This study highlights the need for a speech and language therapist to consider and explore parents' perspectives of their child's SLCN as a part of achieving collaboration with a parent in order to achieve the best outcomes. Drawing on the creative group and individual interviews with young people aged 11–18 years old in England and Wales in 2018, this paper assembles data that addresses the changing nature of intimacies in young people's digitally-networked lives. Informed by Lauren Berlant's work (2008, viii), it maps how platforms such as Instagram and YouTube circulate certain 'fantasies of the good life' that orient young people towards heteronormative future imaginaries. It argues that publicising intimate relationships on social media works to both reify and unsettle fantasies of monogamy, marriage and domesticity by highlighting their inherent precarity. It also illustrates how smart technologies such as the virtual personal assistants Alexa and Siri reconfigure the good life fantasy. Moving beyond simplistic moral panics that seek to discipline young people's digital intimacies, this paper aims to engage with the nuances of young people's digital sexual cultures. Overall, this paper argues that attending to this complexity can enable practitioners to better attune to and support young people to navigate contemporary digital relationships and sexuality issues. In this paper, we present evidence of estimated significant associations between greenspace and prisoners' self-reported well-being, self-harm and violence in prisons in England and Wales. Refining and extending our previous research that estimated the relationship between greenspace and self-harm and violence while controlling for the effects of prison characteristics (e.g. prison size, over-crowding and security level), the findings in the present study show that greenspace remains significantly related to self-harm and violence when we additionally control for prison population characteristics (such as prisoner age, ethnicity, sentence length) and when we use additional self-reported indicators of well-being. Furthermore, our findings also show that the beneficial effects of greenspace appear to be particularly prominent in prison establishments that suffer from overcrowding or hold relatively large shares of younger and un-sentenced prisoners. Finally, our results reveal that greenspace has important impacts on the inter-relationships between self-reported well-being, self-harm and prison violence. [ABSTRACT FROM AUTHOR]

Background: Equitable access to healthcare is a priority of many healthcare systems, aiming to ensure access is driven by need and not minority groups such as those defined by sexual orientation. However, there are healthcare areas where inequity in access across sexual orientation groups is found that are not justified based on need. Mandated LGBTQ+-specific training of the healthcare workforce may help address some barriers of access for these groups. The study aims to understand the potential economic implications for mandated LGBTQ+-specific healthcare training on the healthcare system in England, UK to inform commissioning of training provision. Methods: Cervical cancer screening was used as an exemplar case where there appears to be inequity in access for different sexual orientation groups. A decision model was developed and analysed that considered the impacts of greater uptake of screening for lesbian and bisexual women due to LGBTQ+ training. Costs took the perspective of the healthcare system and outcomes modelled were cancer cases averted in a timeframe of 5 years. Results: Based on cervical cancer screening alone, where training costs are fully attributed to this service, training would likely result in fewer cancer cases detected in the lesbian and bisexual populations, though this comes at a modest increase in healthcare sector costs, with this increase largely reflecting a greater volume of screens. Training costs do not appear to be a major component of the cost implications. Conclusions: In resource-constrained systems with increasing pressures for efficiency savings, the opportunity cost of delivering training is a realistic component of the commissioning decision. The findings in this paper provide a signal that mandated LGBTQ+ training in healthcare could lead to potentially greater outcomes and in breaking down barriers of access and could also enable the healthcare system to provide more equitable access to healthcare.
