ObjectiveTo develop a prioritisation framework to support priority setting for elective surgeries after COVID-19 based on the impact on patient well-being and cost.DesignWe developed decision analytical models to estimate the consequences of delayed elective surgical procedures (eg, total hip replacement, bariatric surgery or septoplasty).SettingThe framework was applied to a large hospital in the Netherlands.Outcome measuresQuality measures impacts on quality of life and costs were taken into account and combined to calculate net monetary losses per week delay, which quantifies the total loss for society expressed in monetary terms. Net monetary losses were weighted by operating times.ResultsWe studied 13 common elective procedures from four specialties. Highest loss in quality of life due to delayed surgery was found for total hip replacement (utility loss of 0.27, ie, 99 days lost in perfect health); the lowest for arthroscopic partial meniscectomy (utility loss of 0.05, ie, 18 days lost in perfect health). Costs of surgical delay per patient were highest for bariatric surgery (€31/pp per week) and lowest for arthroscopic partial meniscectomy (−€2/pp per week). Weighted by operating room (OR) time bariatric surgery provides most value (€1.19/pp per OR minute) and arthroscopic partial meniscectomy provides the least value (€0.34/pp per OR minute). In a large hospital the net monetary loss due to prolonged waiting times was €700 840 after the first COVID-19 wave, an increase of 506% compared with the year before.ConclusionsThis surgical prioritisation framework can be tailored to specific centres and countries to support priority setting for delayed elective operations during and after the COVID-19 pandemic, both in and between surgical disciplines. In the long-term, the framework can contribute to the efficient distribution of OR time and will therefore add to the discussion on appropriate use of healthcare budgets. Item does not contain fulltext, Objectives To determine if cone beam CT-guided navigation bronchoscopy (CBCT-NB) is a cost-effective diagnostic procedure in patients with a pulmonary nodule (PN) with an intermediate risk for lung cancer. Materials and methods Two decision analytical models were developed to compare the long-term costs, survival and quality of life. In the first model, CBCT-NB was compared with CT-guided transthoracic needle biopsy (TTNB) in TTNB eligible patients. In the second model, CBCT-NB was compared with direct treatment (without pathology proven lung cancer) in patients for whom TTNB is not suitable. Input data were gathered in-house, from literature and expert opinion. Effects were expressed in quality-adjusted life years (QALYs). Sensitivity analyses were used to assess uncertainty. Results CBCT-NB can be cost-effective in TTNB eligible patients with an incremental cost-effectiveness ratio of euro18 416 in an expert setting. The probabilistic sensitivity analysis showed that in 69% and 90% of iterations CBCT-NB remained cost-effective assuming a willingness to pay (WTP) of euro20 000 and euro80 000 per QALY. CBCT-NB dominated in the treatment strategy in which TTNB is not suitable. The probabilistic sensitivity analysis showed that in 95% of iterations CBCT-NB remained the dominant strategy, and CBCT-NB remained cost-effective in 100% of iterations assuming a WTP limit of euro20 000. In the comparison between CBCT NB and TTNB, the deterministic sensitivity analysis showed that the diagnostic properties and costs of both procedures have a large impact on the outcome. Conclusions CBCT-NB seems a cost-effective procedure when compared with TTNB and when compared with a direct treatment strategy in patients with an intermediate risk PN.

Item does not contain fulltext, Cerebral palsy (CP) is the most common cause of motor disability in children. The largest group of children with CP present with spasticity. Dystonia is estimated to be present in approximately 15% of children with CP, referred to as dyskinetic CP. Still, dystonia in CP remains underdiagnosed. Dystonia and spasticity can occur together in a subgroup of children with CP as well. Dystonia is characterized by fluctuating hypertonia and involuntary movement and postures. Dystonia in children with CP can interfere with motor function, caregiving and comfort. It is important to recognize dystonia in children with CP as specific treatment is indicated. In this paper we describe three cases of children with dystonia in CP and we review the pharmacological treatment options for dystonia in CP and the surgical options including intrathecal baclofen pump and deep brain stimulation.

Contains fulltext : 231725.pdf (publisher's version ) (Open Access), OBJECTIVE: We describe a third patient with brain small vessel disease 3 (BSVD3), being the first with a homozygous essential splice site variant in the COLGALT1 gene, with a more severe phenotype than the 2 children reported earlier. METHODS: Analysis of whole exome sequencing (WES) data of the child and parents was performed. We validated the missplicing of the homozygous variant using reverse transcription PCR and Sanger sequencing of the mRNA in a lymphocyte culture. RESULTS: The patient presented antenatally with porencephaly on ultrasound and MRI. Postnatally, he showed a severe developmental delay, refractory epilepsy, spastic quadriplegia, and a progressive hydrocephalus. WES revealed a homozygous canonical splice site variant NM_024656.3:c.625-2A>C. PCR and Sanger sequencing of the mRNA demonstrated that 2 cryptic splice sites are activated, causing a frameshift in the major transcript and in-frame deletion in a minor transcript. CONCLUSIONS: We report a third patient with biallelic pathogenic variants in COLGALT1, confirming the role of this gene in autosomal recessive BSVD3.

- Abstract
Mitochondrial disorders are clinically heterogeneous and comprise over 350 different genetic conditions. However, the molecular diagnosis is unknown in ~50% of cases, partly due to some genomic regions being refractory to standard genomic analysis. One such region is the ATAD3 locus consisting of 3 highly homologous tandemly arrayed genes (ATAD3C, ATAD3B and ATAD3A) encoding mitochondrial proteins implicated in processes including cholesterol metabolism, and mitochondrial replication, dynamics and morphology. Recessive deletions and dominant duplications in this locus have recently been reported to cause rare, lethal perinatal mitochondrial disorders characterised by pontocerebellar hypoplasia or cardiomyopathy, respectively. We report 17 subjects from 16 unrelated families with cardiomyopathy, persistent hyperlactacidemia, encephalopathy and frequently corneal clouding or cataracts due to de novo ATAD3 duplications. The six different 68 Kb duplications were consistently identifiable from whole genome and exome sequencing, but usually missed on microarray. The duplications all resulted in the formation of an identical chimeric ATAD3A/ATAD3C fusion protein, which appears to act in a dominant manner causing altered ATAD3 complexes and a striking reduction in mitochondrial oxidative phosphorylation complex I and its activity in heart tissue. In our experience, the ATAD3 locus is one of the five most common causes of nuclear-encoded paediatric mitochondrial disease but the repetitive nature of the locus means ATAD3 diagnoses may be frequently missed by current genomic strategies.

Contains fulltext : 232362.pdf (Publisher’s version ) (Open Access), OBJECTIVE: To assess the cost-effectiveness of sentinel lymph node mapping compared to risk factor assessment and routine full lymph node dissection for the assessment of lymph nodes in patients with low- and intermediate-risk endometrioid endometrial cancer. METHODS: A decision-analytic model was designed to compare three lymph node assessment strategies in terms of costs and effects: 1) sentinel lymph node mapping; 2) post-operative risk factor assessment (adjuvant therapy based on clinical and histological risk factors); 3) full lymph node dissection. Input data were derived from systematic literature searches and expert opinion. QALYs were used as measure of effectiveness. The model was built from a healthcare perspective and the impact of uncertainty was assessed with sensitivity analyses. RESULTS: Base-case analysis showed that sentinel lymph node mapping was the most effective strategy for lymph node assessment in patients with low- and intermediate-risk endometrial cancer. Compared to risk factor assessment it was more costly, but the incremental cost effectiveness ratio stayed below a willingness-to-pay threshold of €20,000 with a maximum of €9637/QALY. Sentinel lymph node mapping was dominant compared to lymph node dissection since it was more effective and less costly. Sensitivity analyses showed that the outcome of the model was robust to changes in input values. With a willingness-to-pay threshold of €20,000 sentinel lymph node mapping remained cost-effective in at least 74.3% of the iterations. CONCLUSION: Sentinel lymph node mapping is the most cost-effective strategy to guide the need for adjuvant therapy in patients with low and intermediate risk endometrioid endometrial cancer.

Pollutant emissions of road vehicles have reduced significantly thanks to the development and application of effective and often complex emissions control systems. Tampering of these systems leads to elevated emissions levels comparable to uncontrolled levels of vehicles of decades ago. Therefore, a small share of tampering potentially leads to a significant increase of the EU fleet average emissions. For task 3.1 of DIAS, a market assessment was conducted to determine the market of tampering in terms of size, appearance and involved players, to reveal the motivations for tampering and to identify the different types of tampering offered. The exercise has led to a proposal for a test matrix of vehicle – tampering combinations that poses the largest environmental risk and which should be tested in the next project phase to determine the current vulnerabilities and exploits of vehicles that need to be addressed by the DIAS concept. The following conclusions can be drawn: There is a substantial market where tampering is offered for both light‐ and heavy duty vehicles and non‐road mobile machinery. However, not much quantitative information is available which indicates the magnitude of the problem, i.e. the number of vehicles that are tampered in the EU. An important source that does indicate tampering in the EU are road side inspections for trucks were significant tampering rates were found for previous generations of heavy duty vehicles for which an advanced emissions control system is required (Euro IV and V). The inspections are however often selective, targeting specific vehicles based on experience and assumptions, most likely resulting in biased data. The magnitude of the problem is largely unknown for Euro VI and the light duty and non‐road mobile machinery segments. The motive mentioned most for tampering is to avoid costs for repair of malfunctions of the emissions control systems of diesel engines. Other motives mentioned are: costs for consumables, costs for downtime, performance tuning and exhaust sound level. Emissions control systems with higher rates of malfunctions and related costs for repair may therefore pose the largest environmental risk: SCR (Selective Catalytic Reduction), DPF (Diesel Particle Filter), EGR (Exhaust Gas Recirculation) for diesel engines but possibly also TWC (Three‐Way Catalyst) for older gasoline engines

Item does not contain fulltext, Several de novo variants in the KIF1A gene have been reported to cause a complicated form of hereditary spastic paraplegia. Additional symptoms include cognitive impairment and varying degrees of peripheral neuropathy, epilepsy, decreased visual acuity, and ataxia. We describe four patients (ages 10-18 years), focusing on their mobility and gait characteristics. Two patients were not able to walk without assistance and showed a severe abnormal gait pattern, crouch gait. At examination, severe contractures were found.In addition to describing the different phenotypes with specific attention to gait in our cases, we reviewed known KIF1A mutations and summarized their associated phenotypes.We conclude that mobility and cognition are severely affected in children with spastic paraplegia due to de novo KIF1A mutations. Deterioration in mobility is most likely due to progressive spasticity, muscle weakness, and the secondary development of severe contractures, possibly combined with an additional progressive polyneuropathy. Close follow-up and treatment of these patients are warranted.

TNO actualiseert jaarlijks de emissiefactoren voor wegverkeer.Deze emissiefactoren worden gebruikt in de Emissieregistratie, onder meer ten behoeve van internationale emissierapportages, maar ook in verspreidingsmodellen ten behoeve van de berekening van concentraties van luchtverontreinigende stoffen.In 2019 is een aantal emissiefactoren wegverkeer bijgesteld op basis van nieuwe inzichten uit recent onderzoek: - Oudere benzineauto’s hebben incidenteel problemen met de emissiecontroletechnologie, wat een grote toename in gemiddelde NOx emissies tot gevolg heeft. - De modernste Euro VI vrachtwagensfunctioneren minder goed bij lage snelheden met als gevolg hogere NOx emissies in de stad. - Daarnaast is er een inschatting gemaakt van het effect van het (op beperkte schaal) verwijderen van roetfilters op fijnstof (PM) en EC emissies van moderne diesel personenauto’s en bestelauto’s. - Verder zijn de NH3 emissiefactoren aangepast om zo de geobserveerde slip van ammonia uit de katalysator in Euro VIvrachtwagens goed te reflecteren. Op verschillende details zijn de wagenparkontwikkelingen aangepast voor 2020. Voor andere zichtjaren zijn er geen veranderingen in het wagenpark doorgevoerd. Voor 2014 en 2030 is er geen verandering in het wagenpark verondersteld ten opzichte van eerdere inzichten, omdat er geen nieuwe gegevens beschikbaar zijn. Voor milieuzones vrachtverkeer in de stad wordt dit jaar uitgegaan van 10% overtredingen en ontheffingen, ten opzichte van de eerdere inschatting van 25%. Er zijn voor 2019 door PBL geen nieuwe ramingen gemaakt van de totale emissies van transport, die naast wegverkeer ook emissies van binnenvaart, bromfietsen, en mobiele werktuigen bevat. Het effect van veranderingen in de huidige emissies van transport zijn voor volledigheidshalve in dit rapport opgenomen.

In the Dutch in-service emissions testing programme, the nitrogen oxide (NOx) emissions of 46 vehicles with Euro VI certified engines were measured during normal daily operation using the Smart Emission Measurement System (SEMS). To screen the in-service conformity, a number of vehicles were tested over the prescribed in-service conformity test routes using SEMS and in some cases the Portable Emission Measurement System (PEMS). All the screened vehicles showed indicative good results over the in-service conformity routes using the pass-fail evaluation method. In daily operation however, the NOx emissions vary a lot from case to case and are often not as low as one might expect based on the EU standard. The variation in NOx emissions is largely caused by the spread in operation profiles between the vehicles. Average speeds vary and especially at low speed, low load the probability that NOx emissions increase is higher. The cause is the fact that in these conditions the NOx abatement technology that is present on Euro VI step A to C certified engines is not always fully effective.

In dit programma meet TNO, op regelmatige basis, de uitlaatgasemissies van deze voertuigen om te onderzoeken hoe schoon ze in de praktijk zijn en of ze aan de Europese normen voldoen. Euro-VI voertuigen vertegenwoordigen inmiddels een groot en snel groeiend aandeel in de Nederlandse vloot. Sinds de inwerkingtreding van Euro VI op 1 januari 2014, richt het meetprogramma zich dan ook voornamelijk op het meten en monitoren van de praktijkemissies van hoofdzakelijk dieselvoertuigen die aan deze norm voldoen. In dit rapport wordt verslag gedaan van de resultaten van het emissiemeetprogramma vrachtwagens 2015 - 2018 waarbij een overzicht wordt gegeven van alle tot dusver gemeten voertuigen met een Euro VI gecertificeerde motor, waaronder vrachtwagens, bussen, vuilniswagens en een enkel speciaal voertuig

In het kader van het ‘Steekproefcontroleprogramma voor vrachtwagens en bussen’ voor het Ministerie van Infrastructuur en Waterstaat zijn door TNO praktijkemissietesten uitgevoerd aan een lange-afstandsvrachtwagen met een Euro-VI tweebrandstoffenmotor (LNG-diesel) om het niveau van de uitstoot van schadelijke stoffen en de uitstoot van broeikasgassen te bepalen. Het voertuig werd geïntroduceerd in 2018 en vertegenwoordigt daarmee de nieuwste LNG technologie voor vrachtwagens voor de lange afstand. De resultaten worden in dit rapport gepresenteerd naast eerder in het kader van het programma onder vergelijkbare Nederlandse rijcondities verkregen emissieresultaten van een groep soortgelijke trucks met Euro VI dieselmotoren en motoren die enkel op LNG draaien. Voor het bepalen van de broeikasemissies zijn naast de CO2 emissies uit de uitlaat ook de CH4 emissies uit de uitlaat gemeten. Tijdens het meetprogramma is ook de boil-off uit de tank gemeten en gemonitord om een indicatie te krijgen van de bijdrage van deze bronnen aan de broeikasgasemissie op oertuigniveau. Er zijn zeer indicatieve metingen gedaan aan de uitstoot van het sterke broeikasgas N2O. Aanvullend zijn formele testen uitgevoerd om de conformiteit van het in gebruik zijnde voertuig te controleren ten aanzien van de gereguleerde uitstoot van schadelijke gasvormige stoffen.

Dit aantal is te laag om algemeen geldende conclusies te trekken over het praktijkemissieniveau van vuilnisauto’s met een Euro VI aardgasmotor.

Sinds Euro VI van kracht is, moeten voor de certificering van een heavy-duty motor of voertuig (GVW>3.5t) verplicht praktijkemissietesten worden uitgevoerd. De invoering van dit type test heeft samen met enkele andere belangrijke aanpassingen in de EU emissiewetgeving geleid tot een forse afname van de gemiddelde NOX emissies van zware bedrijfswagens. Voor een aantal toepassingen was het beeld echter nog wisselend, met name onder stadse rijcondities zijn de NOX emissies regelmatig verhoogd [TNO, 2016], [TNO, 2018]. De praktijkemissietest is bedoeld om de NOX emissies te controleren onder zogenaamde ‘normale gebruiksomstandigheden' van het voertuig. De test heeft grenscondities waarbuiten NOX emissies niet worden gecontroleerd. Een analyse is uitgevoerd om te bepalen welke condities, die in Nederland typisch kunnen voorkomen, buiten de condities van de test vallen waardoor de NOX emissies in principe ongecontroleerd blijven en er een risico is op verhoogde NOX emissies. Nederland kan deze informatie gebruiken om in Brussel te pleiten voor verbetering van de praktijktestprocedure.

Daarnaast zijn twee Euro VI bussen uitgebreid op de openbare weg doorgemeten met het meetsysteem ‘PEMS’ over de voor de typegoedkeuring en de conformiteitstest voorgeschreven busrit en ook over enkele representatieve buslijnen. Alle zeven bussen hebben een eerste generatie Euro VI dieselmotor met een SCR katalysator voor de reductie van de NOx emissies en een roetfilter voor de reductie van de fijnstofuitstoot van de dieselmotoren. De motoren van de geteste bussen representeren 64% van de geregistreerde typen Euro VI motoren in OV bussen in Nederland.

34. On-road emission measurements with PEMS on a Mercedes-Benz Atego Euro VI N2 heavy-duty truck

Commissioned by the Ministry of Infrastructure and Water management of The Netherlands, TNO Sustainable Transport and Logistics regularly performs measurements on heavy-duty vehicles to determine the in-service performance and durability of the tail-pipe emissions under representative driving conditions. A Euro VI heavy-duty N2 class truck, the Mercedes-Benz Atego was tested with a Portable Emission Measurement System on the road (PEMS), driving various trips. The gaseous emissions were measured and analysed according to the in-service conformity method (Annex II of 582/2011/EC and amendments). With the standard PEMS protocol adapter no access to CAN/ECU data was possible. Therefore, for all but one test only the CO2 based evaluation method was chosen. Because of the same reason for these test the engine coolant temperature was not available and therefore a warm up time of the coolant had to be assumed for these tests. For one of the tests CAN/ECU data was recorded with a separate device. The data (torque, engine speed, coolant temperature) was used for the work-based evaluation of the given test. It should be noted that the share of the sub-trip part characterized as ‘rural’ is smaller than required for an N2 trip according regulation 582/2011 amended by 64/2012. The test trips are therefore not fully conform the applicable requirements. The conclusions drawn from the measurements with this specific vehicle are as follows: - the Conformity Factor for NOx over an N2 Euro VI trip is high and well above 1.5. - the Conformity Factor for NOx varies differs per trip and varies between 3.4 and 5.8 for trips with a payload of 55%. - the Conformity Factors for CO and HC emission are well below 1.5 for all trips. Generally, for in-service conformity checking, more than one vehicle should be tested and evaluated to determine whether the vehicle type is compliant with the in-service conformity requirements. In this programme only one vehicle was tested and therefore the results are indicative only.

Background: Children with severe cerebral palsy (CP) (i.e. 'non-speaking children with severely limited mobility') are restricted in many domains that are important to the acquisition of language. Aims: To investigate comprehension of spoken language on sentence type level in non-speaking children with severe CP. Methods & Procedures: From an original sample of 87 non-speaking children with severe CP, 68 passed the pre-test (i.e. they matched at least five spoken words to the corresponding objects) of a specifically developed computer-based instrument for low motor language testing (C-BiLLT), admitting them to the actual C-BiLLT computer test. As a result, the present study included 68 children with severe CP (35 boys, 33 girls; mean age 6;11 years, SD 3;0 years; age range 1;9-11;11 years) who were investigated with the C-BiLLT for comprehension of different sentence types: phrases, simple active sentences (with one or two arguments) and compound sentences. The C-BiLLT provides norm data of typically developing (TD) children (1;6-6;6 years). Binomial logistic regression analyses were used to compare the percentage correct of each sentence type in children with severe CP with that in TD children (subdivided into age groups) and to compare percentage correct within the CP subtypes. Outcomes & Results: Sentence comprehension in non-speaking children with severe CP followed the developmental trajectory of TD children, but at a much slower rate; nevertheless, they were still developing up to at least age 12 years. Delays in sentence type comprehension increased with sentence complexity and showed a large variability between individual children and between subtypes of CP. Comprehension of simple and syntactically more complex sentences were significantly better in children with dyskinetic CP than in children with spastic CP. Of the children with dyskinetic CP, 10-13% showed comprehension of simple and compound sentences within the percentage correct of TD children, as opposed to none of the children with spastic CP. Conclusion & Implications: In non-speaking children with severe CP sentence comprehension is delayed rather than deviant. Results indicate the importance of following comprehension skills across all age groups, even beyond age 12 years. Moreover, the subtype of CP should be considered when establishing an educational programme for sentence comprehension, and augmentative and alternative communication support. In addition, educational programmes for children with severe CP should take into account the linguistic hierarchy of sentence comprehension when focusing on the input and understanding of spoken language comprehension.

- Abstract
Introduction: Acquired demyelinating syndromes (ADS) are immune-mediated demyelinating disorders of the central nervous system in children. A nationwide, multicentre and prospective cohort study was initiated in the Netherlands in 2006, with a reported ADS incidence of 0.66/100,000 per year and MS incidence of 0.15/100,000 per year in the period between 2007 and 2010. In this study, we provide an update on the incidence and the long-term follow-up of ADS in the Netherlands. Methods: Children < 18 years with a first attack of demyelination were included consecutively from January 2006 to December 2016. Diagnoses were based on the International Paediatric MS study group consensus criteria. Outcome data were collected by neurological and neuropsychological assessments, and telephone call assessments. Results: Between 2011 and 2016, 55/165 of the ADS patients were diagnosed with MS (33%). This resulted in an increased ADS and MS incidence of 0.80/100,000 per year and 0.26/100,000 per year, respectively. Since 2006 a total of 243 ADS patients have been included. During follow-up (median 55 months, IQR 28–84), 137 patients were diagnosed with monophasic disease (56%), 89 with MS (37%) and 17 with multiphasic disease other than MS (7%). At least one form of residual deficit including cognitive impairment was observed in 69% of all ADS patients, even in monophasic ADS. An Expanded Disability Status Scale score of ≥ 5.5 was reached in 3/89 MS patients (3%). Conclusion: The reported incidence of ADS in Dutch children has increased since 2010. Residual deficits are common in this group, even in monophasic patients. Therefore, long-term follow-up in ADS patients is warranted.

In het kader van het in-service testprogramma voor vrachtauto’s en bussen voor het Ministerie van Infrastructuur en Waterstaat zijn emissiemetingen uitgevoerd aan twee Euro VI vrachtauto’s op vloeibaar aardgas (liquefied natural gas, LNG). De twee geteste vrachtauto’s vertegenwoordigen de huidige stand (2017) van de LNG-technologie van in Nederland verkochte lange-afstandstrucks. In metingen op de weg zijn de emissies bepaald van luchtverontreinigende stoffen en broeikasgassen, en is het praktijk-brandstofverbruik vastgesteld. De resultaten worden vergeleken met die van eerder in dit kader geteste Euro VI dieselvrachtauto’s. Alle testen zijn uitgevoerd onder vergelijkbare, Nederlandse praktijkcondities. Naast de gemeten CO2-emissies uit de uitlaat zijn ook de directe emissies van andere broeikasgassen in een beknopte scenarioanalyse ingeschat.

Begin 2017 kwam via de media naar voren dat manipulatie plaatsvindt van systemen die op vrachtwagens zitten om de schadelijke uitlaatgasemissies terug te dringen. Het manipuleren houdt in dat in een vrachtwagen een kastje wordt ingebouwd of de motorsoftware wordt aangepast zodat het emissiebeheersingssysteem wordt uitgeschakeld. Het uitschakelen van emissiebeheersingssystemen wordt volgens Europese verordening (EG) nr. 595/2009/EC gezien als manipulatie en is verboden. Uit een marktverkenning blijkt dat producten en diensten waarmee emissiebeheersingssystemen kunnen worden gedeactiveerd gemakkelijk te verkrijgen zijn. Door het emissiebeheersingssysteem te (laten) manipuleren, vermijdt een transporteur illegaal onder meer de kosten voor het verbruiksmiddel AdBlue dat nodig is om het emissiebeheerssysteem te kunnen laten werken. Door het uitschakelen van het systeem bespaart de eigenaar ongeveer 1.000 EUR per jaar per truck en neemt de NOx emissie toe met een factor 4 tot 12. Vanaf 2005 zijn de meeste trucks van een emissiebeheersingssysteem voorzien en is het risico van manipulatie reëel.

Dit rapport beschrijft de resultaten van metingen aan de praktijkemissies van Euro 5 en Euro 6/VI dieselbestelauto’s en busjes voor personenvervoer. Praktijkemissies zijn emissies zoals deze optreden, rijdend met het voertuig, op de openbare weg. Het project waarbinnen deze metingen zijn verricht wordt door TNO uitgevoerd in opdracht van het Nederlandse Ministerie van Infrastructuur en Waterstaat. TNO heeft dit soort metingen eerder uitgevoerd aan Euro 6 dieselpersonenauto’s en aan Euro 5 dieselbestelauto’s [TNO 2016c, TNO 2016d]. Net als bij Euro 6 personenauto’s is gebleken dat de NOx-uitstoot van Euro 6/VI dieselbestelauto’s in de praktijk gemiddeld flink hoger is dan de NOx-uitstoot gemeten tijdens de officiële typekeuringstest op de rollenbank. Voor Euro 6 liggen de NOx-emissies van bestelauto’s in absolute zin wel op een lager niveau dan voor Euro 5. Er blijkt een grote spreiding te zijn in de NOx-emissies van de geteste voertuigen. Een aantal van de geteste Euro 6/VI bestelauto’s liet in de praktijk NOx-emissies zien die op of onder de voor de typekeuring geldende grenswaarden liggen. Ook de CO2-uitstoot is gemeten. Gebleken is dat de CO2-uitstoot in praktijktesten hoger ligt dan de typekeurwaarde

Background: Brain MRI is a useful tool for diagnosing inflammatory demyelinating disorders in children. However, it remains unclear which are the most reliable criteria for distinguishing multiple sclerosis (MS) from monophasic disorders such as acute disseminated encephalomyelitis (ADEM). We therefore compared the 4 current sets of MRI criteria in our Dutch pediatric cohort and determined which are the most useful in clinical practice for distinguishing ADEM from MS.Methods: We included 49 children who had had a demyelinating event and an MRI scan within 2 months of their first clinical attack. Twenty-one patients had ADEM and remained relapse-free after at least 2 years of follow-up. Twenty-eight patients had a definitive diagnosis of MS. We assessed the sensitivity and specificity of the following MRI criteria: Barkhof criteria, KIDMUS criteria, Callen MS-ADEM criteria, and Callen diagnostic MS criteria.Results: The Callen MS-ADEM criteria had the best combination of sensitivity (75%) and specificity (95%). The KIDMUS criteria had higher specificity (100%), but much lower sensitivity (11%). The Barkhof criteria had a sensitivity of 61% and a specificity of 91%. The Callen diagnostic MS criteria were the most sensitive (82%), but were only 52% specific for distinguishing a first attack of MS from ADEM.Conclusions: The results in our cohort demonstrate that the new Callen criteria for multiple sclerosis-acute disseminated encephalomyelitis (MS-ADEM) are the most useful for differentiating a first attack of MS from monophasic ADEM. Although the Callen diagnostic MS criteria are more sensitive, they lack the specificity necessary to differentiate MS from ADEM.

Several de novo variants in the KIF1A gene have been reported to cause a complicated form of hereditary spastic paraplegia. Additional symptoms include cognitive impairment and varying degrees of peripheral neuropathy, epilepsy, decreased visual acuity, and ataxia. We describe four patients (ages 10–18 years), focusing on their mobility and gait characteristics. Two patients were not able to walk without assistance and showed a severe abnormal gait pattern, crouch gait. At examination, severe contractures were found. In addition to describing the different phenotypes with specific attention to gait in our cases, we reviewed known KIF1A mutations and summarized their associated phenotypes. We conclude that mobility and cognition are severely affected in children with spastic paraplegia due to de novo KIF1A mutations. Deterioration in mobility is most likely due to progressive spasticity, muscle weakness, and the secondary development of severe contractures, possibly combined with an additional progressive polyneuropathy. Close follow-up and treatment of these patients are warranted. [ABSTRACT FROM AUTHOR]

Item does not contain fulltext, Background: To report our experience of infections in deep brain stimulation (DBS) surgeries comparing shaving versus no shaving of cranial hair. Nonshaving is strongly preferred by patients due to aesthetic and psychological factors. Methods: This study is a prospective follow-up of the infection rate in 43 nonshaven DBS cases between April 2014 and December 2015 compared to our former infection rate with shaving in our center. Minimum follow-up was 6 months. All patients, except 7 epilepsy patients, received implantation of the electrodes together with the extension cables and internal pulse generator in one session. Results: In 43 nonshaven patients, a total of 81 electrodes were implanted or revised with a mean follow-up of 16 months. One patient (2.32%) developed an infection of the implanted DBS-hardware and was treated with antibiotics. Conclusion: In our experience nonshaving of cranial hair in DBS surgery does not lead to more infections when compared to shaving. We have changed our protocol to nonshaving based on these findings.

- Abstract
Background and purpose: To support clinical decision-making in central neurological disorders, a physical examination is used to assess responses to passive muscle stretch. However, what exactly is being assessed is expressed and interpreted in different ways. A clear diagnostic framework is lacking. Therefore, the aim was to arrive at unambiguous terminology about the concepts and measurement around pathophysiological neuromuscular response to passive muscle stretch. Methods: During two consensus meetings, 37 experts from 12 European countries filled online questionnaires based on a Delphi approach, followed by plenary discussion after rounds. Consensus was reached for agreement ≥75%. Results: The term hyper-resistance should be used to describe the phenomenon of impaired neuromuscular response during passive stretch, instead of for example 'spasticity' or 'hypertonia'. From there, it is essential to distinguish non-neural (tissue-related) from neural (central nervous system related) contributions to hyper-resistance. Tissue contributions are elasticity, viscosity and muscle shortening. Neural contributions are velocity dependent stretch hyperreflexia and non-velocity dependent involuntary background activation. The term 'spasticity' should only be used next to stretch hyperreflexia, and 'stiffness' next to passive tissue contributions. When joint angle, moment and electromyography are recorded, components of hyper-resistance within the framework can be quantitatively assessed. Conclusions: A conceptual framework of pathophysiological responses to passive muscle stretch is defined. This framework can be used in clinical assessment of hyper-resistance and will improve communication between clinicians. Components within the framework are defined by objective parameters from instrumented assessment. These parameters need experimental validation in order to develop treatment algorithms based on the aetiology of the clinical phenomena.

- Abstract
This study aims to (1) investigate the neuropathology of mild to severe pediatric TBI and (2) elucidate the predictive value of conventional and innovative neuroimaging for functional outcome. Children aged 8–14 years with trauma control (TC) injury (n = 27) were compared to children with mild TBI and risk factors for complicated TBI (mildRF+, n = 20) or moderate/severe TBI (n = 17) at 2.8 years post-injury. Neuroimaging measures included: acute computed tomography (CT), volumetric analysis on post-acute conventional T1-weighted magnetic resonance imaging (MRI) and post-acute diffusion tensor imaging (DTI, analyzed using tract-based spatial statistics and voxel-wise regression). Functional outcome was measured using Common Data Elements for neurocognitive and behavioral functioning. The results show that intracranial pathology on acute CT-scans was more prevalent after moderate/severe TBI (65%) than after mildRF+ TBI (35%; p = .035), while both groups had decreased white matter volume on conventional MRI (ps ≤ .029, ds ≥ −0.74). The moderate/severe TBI group further showed decreased fractional anisotropy (FA) in a widespread cluster affecting all white matter tracts, in which regional associations with neurocognitive functioning were observed (FSIQ, Digit Span and RAVLT Encoding) that consistently involved the corpus callosum. FA had superior predictive value for functional outcome (i.e. intelligence, attention and working memory, encoding in verbal memory and internalizing problems) relative to acute CT-scanning (i.e. internalizing problems) and conventional MRI (no predictive value). We conclude that children with mildRF+ TBI and moderate/severe TBI are at risk of persistent white matter abnormality. Furthermore, DTI has superior predictive value for neurocognitive out-come relative to conventional neuroimaging.

In 2012 ondertekende de Minister van het Ministerie van Infrastructuur en Milieu de Green Deal Zero Emissie Busvervoer, met als ultieme doel een volledig emissievrije bussenvloot in 2025. Dat betekent een ambitie om in 2025 alle OV-bussen te hebben voorzien van elektrische aandrijving, waaronder batterij-elektrisch en waterstofelektrisch. Elektrische bussen kennen momenteel een beperktere inzetbaarheid dan conventionele bussen met een dieselverbrandingsmotor. De inzet van elektrische bussen in de aanloop naar 2025 heeft daarom een grote impact op bijvoorbeeld dienstrooster en de total cost of ownership (TCO) van de bussenvloot Voor goede keuzes omtrent de in te zetten bussen is gedegen objectieve informatie over de inzetbaarheid van innovatieve busconcepten onmisbaar. Daarom heeft TNO in opdracht van het Ministerie van Infrastructuur en Milieu in het project ‘Zero Emissie Bussen’ metingen uitgevoerd aan vier typen innovatieve bussen. Het gaat dan om bussen met een hybride aandrijving met en zonder plug-in faciliteit, volledig elektrische bussen en een bus op waterstof. TNO heeft eerst de parameters vastgesteld die de inzetbaarheid van zero emissie bussen bepalen. Daarna zijn van zes bussen deze parameters onder Nederlandse omstandigheden gemeten. Met deze informatie kan TNO uitspraak doen over de inzetbaarheid van de verschillende typen bussen in de Nederlandse praktijk.

Moderne zware Euro VI vrachtwagens stoten in de praktijk gemiddeld een factor 10 minder stikstofoxiden (NOx) uit dan eerdere generaties Euro IV en Euro V vrachtwagens. De emissies van Euro 6 dieselpersonen- en bestelwagens laten echter een heel ander beeld zien. Ondanks voortdurende aanscherping van Europese emissielimieten blijft namelijk de praktijkuitstoot van NOx van nieuwe dieselpersonen- en dieselbestelwagens de afgelopen twee decennia nagenoeg onveranderd. De praktijkuitstoot ligt daarmee fors hoger dan de opeenvolgende Euro-limieten gesteld in de emissiewetgeving. De NOx-uitstoot door auto’s en vrachtauto’s draagt in belangrijke mate aan bij aan de concentratie van het schadelijke NO2 in de lucht. Om aan de Europese normen voor de luchtkwaliteit te voldoen is het terugdringen van de NOx-uitstoot van het Nederlandse wegverkeer van groot belang.
