An altimeter is a critical instrument in planetary missions, for both safe operations and science activities. We present required specifications and link budget calculations for light detection and ranging (LIDAR) onboard the Martian Moons Exploration (MMX) spacecraft. During the mission phase, this LIDAR will continuously measure the distance between the spacecraft and its target. The time-series distance provides important diagnostic information for safe spacecraft operations and important information for geomorphological studies. Because MMX is a sample return mission, its LIDAR must accommodate physical disturbances on the Martian satellite surface. This resulted in changes to the optical system design. Graphical abstract

The present paper proposes a measure for improving the wind-resistant performance of photovoltaic systems and mechanically attached single-ply membrane roofing systems installed on flat roofs by combining them together. Mechanically attached single-ply membrane roofing systems are often used in Japan. These roofing systems are often damaged by strong winds, because they are very sensitive to wind action. Recently, photovoltaic (PV) systems placed on flat roofs have become popular. They are also often damaged by strong winds directed onto the underside, which cause large wind forces onto the PV panels. For improving the wind resistance of these systems, we proposed to install PV panels horizontally with gaps between them. Such an installation may decrease the wind forces on the PV panels due to the pressure equalization effect as well as on the waterproofing membrane due to the shielding effect of the PV panels. This paper discusses the validity of such an idea. The pressure on the bottom surface of a PV panel, called the “layer pressure” here, was evaluated by a numerical simulation based on the unsteady Bernoulli equation. In the simulation, the time history of the external pressure coefficients, measured at many points on the roof in a wind tunnel, was employed. It was found that the wind forces, both on the PV panels and on the roofing system, were significantly reduced. The reduction was large near the roof’s corner, where large suction pressures were induced in oblique winds. Thus, the proposed method improved the wind resistance of both systems significantly.

AIM The new guidelines in Japan do not recommend a vancomycin (VCM) loading dose for patients with an estimated glomerular filtration rate (eGFR) 30 < and ≤ 80 mL×min-1×1.73m-2 (moderate renal dysfunction) or administration to those with the eGFR < 30 mL×min-1×1.73m-2 (severe renal dysfunction). We investigated the safety and efficiency of VCM in patients with moderate and severe renal dysfunction based on the new guidelines. MATERIALS AND METHODS The study involved patients admitted to our hospital between April 2014 and March 2018 with an eGFR < 80 mL×min-1×1.73m-2 and treated with VCM. VCM trough concentration and pre- and post-administration renal function were investigated retrospectively. The primary endpoints were the proportion of patients who achieved an effective trough concentration of 10 - 20 µg/mL and rate of acute kidney injury (AKI). RESULTS We included 64 patients (32 moderate, 32 severe). The mean VCM trough concentration achieved for the first time was 9.3 and 11.6 µg/mL in the moderate and severe renal dysfunction groups, respectively (p = 0.91). The effective trough concentration endpoint was achieved by 50% and 43% of the patients in the severe and moderate renal dysfunction groups, respectively, and no significant difference was found in the AKI rate. The serum creatinine change was significantly different between the groups - the moderate group showed a slight deterioration and the severe renal dysfunction group an improvement. CONCLUSION It may be necessary to increase the dose for these patients with severe renal dysfunction while implementing a VCM loading dose and monitoring trough concentrations and adverse effects.

In monosymptomatic nocturnal enuresis (MNE) treatment, enuretic alarm devices are the first recommended treatment option. This study aimed to compare retrospectively the effectiveness of wearable wireless and wired alarm devices for MNE treatment in children aged 6-14 years.All children aged 6-16 with MNE who underwent alarm therapy as outpatients were included. A wired alarm device was used from 2012 to 2015, and a wireless alarm device was used from 2016 to 2019. The primary outcomes were the dropout rates during therapy and at last follow up. The full response(14 consecutive dry nights) and the partial response rate during therapy were also assessed.Of the 173 patients enrolled, 75 and 98 used a wired and a wireless alarm device, respectively. The dropout rate at the last visit was significantly lower in the wireless alarm group than that in the wired alarm group (6.1% vs. 20.0%; P = 0.006). The full response(FR) rate was significantly higher in the wireless alarm group than these in the wired alarm group at 4, 12, 24 weeks (4 weeks: 11.2% vs. 1.3%, P = 0.011; 12 weeks: 31.9% vs. 13.5%, P = 0.005; 24 weeks: 72.9% vs. 39.7%, P 0.0001).Wireless alarm therapy for MNE had lower attrition rates and a higher rate of FR than wired alarm therapy.

The objective of this study was to investigate the differences in the clinical characteristics of Kawasaki disease between older and younger children.This retrospective study examined 405 children with Kawasaki disease admitted to Showa University Northern Yokohama Hospital between 2015 and 2019.Eligible patients were classified into the older (≥3.0 years of age, n = 169) and younger (3.0 years of age, n = 236) groups. Skin rash was found in significantly fewer cases (112 [66.3%] vs 229 [97.0%], P .001 in the younger group). Cervical lymphadenopathy was more common in older children (153 [90.5%] vs 165 [69.9%], P .001) and in incomplete Kawasaki disease (3 or 4 findings) (34 [20.1%] vs 25 [10.6%], P = .0078). The diagnosis was more delayed in older children (median: 5.0 days vs 4.0 days, P = .003) than the younger group. Additionally, fever nonresponsive to a single intravenous immunoglobulin was more common, and the duration of fever was significantly longer in the older group (48 [28.4%] vs 47 [19.9%], P = .0479).Kawasaki disease should be suspected in children aged3.0 years with cervical lymphadenopathy and fever, despite the absence of skin rash. Additionally, incomplete Kawasaki disease, fever unresolved by a single intravenous immunoglobulin infusion, and the tendency to delay treatment initiation are more common in children aged3.0 years.

Recently, progression of disease within 24 months (POD24) has been demonstrated as a strong prognostic indicator in various types of malignant lymphoma. Peripheral T-cell lymphoma (PTCL) has an aggressive course and poor clinical outcomes. In this multicenter retrospective study, 111 consecutively registered patients with newly diagnosed PTCL were analyzed. Of these patients, 72 (64.9%) experienced POD24 (POD24 group), and the other 39 patients (35.1%) were analyzed as the no POD24 group. In the POD24 group, overall survival (OS) was significantly inferior to all patients, and in the no POD24 group, subsequent OS was significantly superior to the POD24 group, although the clinical characteristics between the POD24 group and no POD24 group were not significantly different. Twenty-three patients (20.7%) showed primary refractory disease to first-line therapy, and the prognosis was poor. The International Prognostic Index score and POD24 were identified as independent predictors in multivariate analysis for OS in all patients, and only performance status was an independent prognostic factor for OS in the POD24 group in multivariate analysis. In conclusion, the clinical significance of assessing POD24 in PTCL and the poor prognosis in patients with early disease progression were demonstrated.

The overall outcome of patients with advanced-stage follicular lymphoma (FL) has improved significantly. However, some patients still develop multiple relapsed/refractory FL (RRFL). To address the still-limited data on this population, we performed this multi-center retrospective study. We analyzed 41 patients who received third-line treatment for RRFL at 8 institutes. The median age at diagnosis was 59 years (range, 38-70). The median progression-free survival (PFS) and probability of PFS at 2 years were 1.61 years and 39.4%, respectively, after third-line chemotherapy, and 0.45 years and 19.0%, respectively, after fourth-line chemotherapy. Objective response (OR) after third-line chemotherapy was achieved in 24 patients (53.7%). Bendamustine (Ben)-based regimens were associated with a significantly higher OR rate than other regimens (77.8% vs. 40.0%, respectively, P = 0.025). The median overall survival (OS) and probability of OS at 2 years were 4.71 years and 65.9%, respectively, after third-line chemotherapy, and 1.01 year and 45.1%, respectively, after fourth-line chemotherapy. In conclusion, this study had a small sample size and retrospective design, but it was able to demonstrate poor response rate and duration in patients with multiple RRFL, particularly after fourth-line chemotherapy. The optimal treatment strategy in this population should be clarified, including possibly hematopoietic stem cell transplantation.

Recently, in the mid-upper reach of Japanese rivers, due to artificial impacts such as dam construction, original gravel bar environments and endemic plants have been disappearing, meanwhile alien plant communities have been expanding. As a countermeasure against these issues, gravel bar restoration project started in Kinugawa River. In the present study, vegetation transition was investigated from 2012 to 2014 on a large-gravel bar with two lane meandering in Kinugawa River, where the restoration had just been implemented. It is shown that U-shaped vegetation zone, open to the upstream side, was formed along the main channel and the second channel. This U-shaped zone was mainly composed of Poaceae. The endemic species for gravel bar, such as Aster kantoensis Kitam and Anaphalis margaritacea subsp. yedoensis, and the alien species, such as Eragrostis curvula, inhabited according to the sediment size deposited in/around the U-shaped zone. In 2015, we had a large flood. This gravel bar was greatly deformed and almost all vegetation was flushed away. A field investigation with UAV in 2018 revealed that the U-shaped zone reappeared and the endemic plant communities were widely enlarged in the similar range of relative height through these seven years. Based on the field investigation, laboratory experiments were performed which examined the effect of the U-shaped zone on flow field and coarse sediment movement. It is suggested that fine sand for Eragrostis curvula can be deposited on the internal or downstream area of the U-shaped zone, and that gravel for the endemic species can appear in the outer edge of the U-shaped zone.

Few studies have evaluated the efficacy of ultrasonography (US) and abdominal radiography in assessing bladder and bowel dysfunction in children aged24 months. We aimed to investigate the association between the risk of urinary tract infection (UTI) recurrence and fecal impaction using imaging findings.The medical records of 121 children (aged24 months) with initial febrile UTI (fUTI) who were admitted to the authors' institution from January 2004 to September 2019 were reviewed retrospectively. We evaluated the rectal diameters of children with suspected fecal impaction that were measured using transabdominal US, or the rectal diameters divided by the distance between the ischial spines that were measured using abdominal radiography. Based on previous reports, we defined fecal impaction as a transabdominal US score of30 mm or an abdominal radiography score of0.5. The definition of functional constipation was based on the child/adolescent Rome IV criteria - i.e., a maximum stool frequency of twice per week.The median age at initial fUTI diagnosis was 4 months. The occurrence of fecal impaction identified via imaging was significantly greater in patients with UTI recurrence than in those without recurrence: yes/no: 17/9 (65.4%) versus 35/60 (36.8%); P = 0.013. On the other hand, the occurrence rates of constipation based on stool frequency did not differ between the two groups. In multiple logistic analyses, fecal impaction detected via imaging was identified as an independent risk factor for fUTI recurrence.Fecal impaction observed via US and abdominal radiography may be useful in predicting the recurrence of fUTI in children.

Background There is little information about Coronavirus Disease 2019 (COVID-19) in children with underlying chronic renal pathologies. Cases report From March until April 15, 2020, 16 children with chronic renal pathologies were diagnosed with COVID-19 in Spain. Of these, 6 had end-stage kidney disease (ESKD) (3 transplant recipients and 3 on chronic hemodialysis). The severity of symptoms was mild in all the patients, with little radiological involvement. Three patients were asymptomatic. Fever and upper respiratory symptoms were the most frequent findings. Basal glomerular filtration worsened in 3 patients; however, recovery was rapidly achieved with rehydration and drug dose adjustment. In 2 patients diagnosed with steroid-dependent nephrotic syndrome, COVID-19 provoked a disease relapse. None required oxygen therapy, and 7 could be managed as outpatients. Conclusions COVID-19 disease appears to have a similar clinical course in children with underlying chronic renal pathologies, even in immunosuppressed cases, as in healthy children of the same age; however, special attention must be paid to fluid management and drug dose adjustment.

Electron tracking based Compton imaging is a key technique to improve the sensitivity of Compton cameras by measuring the initial direction of recoiled electrons. To realize this technique in semiconductor Compton cameras, we propose a new detector concept, Si-CMOS hybrid detector. It is a Si detector bump-bonded to a CMOS readout integrated circuit to obtain electron trajectory images. To acquire the energy and the event timing, signals from N-side are also read out in this concept. By using an ASIC for the N-side readout, the timing resolution of few us is achieved. In this paper, we present the results of two prototypes with 20 um pitch pixels. The images of the recoiled electron trajectories are obtained with them successfully. The energy resolutions (FWHM) are 4.1 keV (CMOS) and 1.4 keV (N-side) at 59.5 keV. In addition, we confirmed that the initial direction of the electron is determined using the reconstruction algorithm based on the graph theory approach. X-ray imaging with computed tomography (CT) is widely used for nondestructive imaging of the interior of the human body. In the next decade, photon-counting X-ray CT is expected to reduce the dose needed and enable multicolor imaging. Recently, we proposed a novel photon-counting method that uses a multipixel photon counter (MPPC), with a significantly high signal gain ( ∼ 10 6 ) and fast temporal response (a few nanoseconds), combined with a high-speed scintillator. To realize photon-counting CT imaging in a wide area irradiated by an extremely high X-ray flux (10 6 -10 7 Hz/mm 2 ), a multichannel MPPC system is required. Thus, we developed a large-scale integrated circuit (LSI) with ultrafast signal-processing capability for use with a 16-channel MPPC. The developed LSI can extract a pulse current from an MPPC array with a large detector capacitance ( ∼ 200 pF) by utilizing an electrical circuit with low input impedance. The LSI offers a high photon count rate above 25 MHz/pixel with noise equal to 1.7 μ A for a dynamic range of ∼ 1.3 mA and an energy resolution of 32 % (FWHM) at 60 keV, thereby enabling ultrafast multicolor CT imaging.

HiZ-GUNDAM is a future satellite mission which will lead the time-domain astronomy and the multi-messenger astronomy through observations of high-energy transient phenomena. A mission concept of HiZ-GUNDAM was approved by ISAS/JAXA, and it is one of the future satellite candidates of JAXA’s medium-class mission. We are in pre-phase A (before pre-project) and elaborating the mission concept, mission/system requirements for the launch in the late 2020s. The main themes of HiZ-GUNDAM mission are (1) exploration of the early universe with high-redshift gamma-ray bursts, and (2) contribution to the multi-messenger astronomy. HiZ-GUNDAM has two kinds of mission payload. The wide field X-ray monitors consist of Lobster Eye optics array and focal imaging sensor, and monitor ~1 steradian field of view in 0.5 – 4 keV energy range. The near infrared telescope has an aperture size 30 cm in diameter, and simultaneously observes four wavelength bands between 0.5 – 2.5 μm. In this paper, we introduce the mission overview of HiZ-GUNDAM.

A high vancomycin trough concentration during therapy is associated with increased nephrotoxicity, and the recent guidelines for therapeutic monitoring of vancomycin recommend target of the ratio of area under the curve (AUC) to minimum inhibitory concentration. We aimed to determine vancomycin trough concentration and AUC that induce nephrotoxicity and evaluate predictive factors associated with a high serum vancomycin trough level according to the initial dosing strategy.We conducted a retrospective cohort study in patients administered intravenous vancomycin from June 2013 to February 2017. Totally, 346 patients were included.38 experienced nephrotoxicity during therapy. The both trough level and AUC were significant risk factors for the occurrence of vancomycin induced-nephrotoxicity (p 0.001, p = 0.001). The exposure-response analysis revealed that the trough level of 15 μg/mL was associated with 12.0% nephrotoxicity incidence and AUC of 600 was associated with 12.9% nephrotoxicity incidence. During the treatment, 90 patients had an initial trough concentration of ≥15 μg/mL, and 124 patients had AUC of ≥600 μg h/mL. The multiple logistic regression analysis revealed body weight (p = 0.001), serum creatinine level (p = 0.028), daily vancomycin dose (p = 0.001), and ICU (p = 0.015) were independent predictive factors for a high trough concentration. And same factors were selected for the high AUC.The risk factors for vancomycin induced nephrotoxicity were comparable in both trough concentration and AUC. The incidence of nephrotoxicity can be reduced by controlling vancomycin trough concentration similarly AUC and promoting antimicrobial stewardship.

Background Serum adiponectin circulates in three multimeric isoforms: high-molecular-weight (HMW), middle-molecular-weight (MMW), and low-molecular-weight (LMW) isoforms. Potential change in the circulating adiponectin levels in patients with nephrotic syndrome (NS) remain unknown. This study aimed to assess the levels of total adiponectin and the distribution of its isoforms in pediatric patients with NS. Methods We sequentially measured total adiponectin and each adiponectin isoform levels at the onset of NS, initial remission, and during the remission period of the disease in 31 NS patients. We also calculated the ratios of HMW (%HMW), MMW (%MMW), and LMW (%LMW) to total adiponectin incuding 51 control subjects. Results The median of total serum adiponectin levels in patients were 36.7, 36.7, and 20.2 μg/mL at the onset, at initial remission, and during the remission period of NS, respectively. These values were significantly higher than those in control subjects. The median values of %HMW, %MMW, and %LMW values were 56.9/27.0/14.1 at the onset, 62.0/21.8/13.4 at the initial remission, and 58.1/21.7/17.5 at during the remission period of NS, respectively. Compared with control subjects, %HMW at initial remission and %MMW at the onset were high, and the %LMW values at the onset and at initial remission were low. Conclusions In patients with NS, total serum adiponectin levels increase at the onset of the disease, and the ratio of adiponectin isoforms changes during the course of the disease. Further studies are needed to delineate the mechanisms between proteinuria and adiponectin isoforms change.

A table-based method for the estimation of heavy-ion-induced Digital Single Event Transient (DSET) voltage pulse-width in a single logic cell has been developed. The estimation method is based on the actual heavy-ion-induced transient current data in a single metal-oxide-semiconductor field effect transistor (MOSFET) used in the logic cell. The DSET pulse waveform in an inverter is obtained from which the pulse-width was estimated to be 420 ps. This DSET pulse-width value (420 ps) falls within the reasonable range of the DSET pulse-width distribution measured by the self-triggering flip-flop latch chain under heavy-ion irradiation test conditions.

BACKGROUND Overactive bladder (OAB) is a symptomatic syndrome defined by urinary urgency, usually accompanied by increased urination frequency and nocturia, with or without urinary incontinence. The prevalence of pediatric OAB in 5-13 year olds is as high as 16.6%, but the pathophysiology and epidemiology have not been sufficiently elucidated. METHODS We retrospectively reviewed medical records in 117 children with OAB aged between 5 and 15 years during the years 2012-2016. At initial presentation, abdominal ultrasound and uroflowmetry were performed, and behavioral modifications, such as timed voiding, and constipation therapy were initiated. If there was no response after 4 weeks, antimuscarinic treatment was added. We evaluated the clinical features of OAB and factors related to the recovery period, which was defined as the period from the start of behavioral modifications to cure. RESULTS The average recovery period was 11.9 ± 9.73 months. There was no significant difference in the recovery period according to age, gender, percentage of urination frequency, nocturnal enuresis, or constipation. The recovery period was significantly shorter in the group with bladder wall thickness ≥5 mm than with bladder wall thickness

Background To assess the health-related quality of life (HRQOL) of children with daytime urinary incontinence (DUI) based on pre- and post-treatment self-reports and parent proxy-reports. Methods The study population comprised 117 children with at least one episode of DUI per week and their caregivers as well as 999 healthy children (control group). The Pediatric Quality of Life Inventory 4.0 (PedsQL) questionnaire was administered to assess the HRQOL of children. To assess the degree of improvement in HRQOL, we categorized children into two groups: group A achieved complete response (CR) to treatment within 12 months and group B did not achieve CR within 12 months. CR was defined as the complete resolution of symptoms or alleviation of symptoms to Results Valid responses were collected from 84 children [53 boys and 31 girls; mean age: 7.9 ± 1.5 years (range, 6–12)]. Sixty-two patients (73.8%) were classified into group A and 22 (26.1%) into group B. Based on self-reports, significant post-treatment improvement was observed in the scores of all PedsQL items (mean total score: 82.2 ± 11.3 vs. 87.2 ± 9.8; P = 0.003). Group A showed significant improvement in the scores of all PedsQL items after achievement of CR based on child self-reports; however, this was improvement not observed in group B. Conclusions To the best of our knowledge, this is the first study to demonstrate the complete resolution of DUI with treatment for improving the HRQOL of these children.

The muon anomalous magnetic moment $(g-2)_{\mu}$ and electric dipole moment (EDM) are sensitive to new physics beyond the Standard Model. We aim to measure $(g-2)_{\mu}$ with a precision of 0.1 parts per million and search for EDM with a sensitivity of $10^{-21}~e \cdot$cm at J-PARC. In this experiment, we use a silicon strip detector with high granularity and fast response to detect positrons from muon decay. We report on the development status of the silicon strip detector.

A high-intensity pulsed muon beam is becoming available at the at the Japan Proton Accelerator Research Complex (J-PARC). Many experiments to study fundamental physics using this high-intensity muon beam are proposed. An experiment to measure the muon magnetic moment anomaly ($g-2$) and the muon electric dipole moment (EDM) is one of these experiments and it requires a tracking detector for positrons from muon decay. Fine segmentation is required in a detector to tolerate the high rate of positrons. The time resolution is required to be much better than the muon anomalous spin precession period while a buffer depth of a front-end electronics needs to be much longer than the accelerated muon lifetime. Requirements of this detector also meet requirements of a measurement of the muonium hyperfine structure interval at the J-PARC and another experiment to measure the proton charge radius at Tohoku University. We have developed a single-sided silicon strip sensor with a 190 $\mu$m pitch, a front-end electronics with a sampling rate of 200 MHz and a buffer memory depth of 8192, and a data acquisition system based on DAQ-Middleware for the J-PARC muon $g-2$/EDM experiment. We have fabricated detector modules consisting of this sensor and the front-end electronics. Performance of fabricated detector modules was evaluated at a laboratory and a beam test using the positron beam at Tohoku University. The detector is confirmed to satisfy all requirements of the experiments except for the time walk, which will be solved by the next version of a front-end electronics., Comment: 18 pages, 17 figures

X-ray computed tomography (CT) is widely used for non-invasive diagnostic imaging of the inside of the human body. It should be noted that the approximate effective radiation dose in a patient is 10 mSv. Under such an environment, X-ray photons are severely piled-up. Therefore, conventional CT only reconstructs energy integrated image, which may consist of beam hardening artifacts that have proven to be a problem. In contrast, photon counting CT (PC-CT) offers a low-dose multicolor CT imaging. The PC-CT also enables K-edge imaging that can improve the blood–tissue contrast using specific contrast agents. Moreover, the PC-CT has great advantages in (1) the simultaneous imaging of multiple contrast agents, and (2) the absolute quantification of contrast agents. Owing to these advantages, the PC-CT system can provide more detailed tissue diagnosis than conventional CT systems. Recently, we proposed a novel PC-CT system (Morita et al., 2017; Arimoto et al., 2017, Maruhashi et al., 2018) consisting of multipixel photon counter (MPPC) coupled with a high-speed scintillator, which is a cost-effective and easy to assemble system, as compared to other PC-CT devices based on cadmium zinc telluride. In this paper, we operated the K-edge imaging of specific contrast agents using a 16-channel MPPC PC-CT system. Our PC-CT system established appropriate energy thresholds and operated the simultaneous imaging of multiple contrast agents such as iodine and gadolinium. In addition, we estimated the absolute concentration of these contrast agents. The results show that our PC-CT system can provide more accurate diagnostic medical imaging, as compared to the conventional CT system.
