Abstract

Resume Le caractere localise de la douleur neuropathique est l’un des elements d’evaluation permettant d’orienter le choix du traitement de premiere intention. Les recommandations internationales preconisent l’utilisation de traitements locaux, comme les emplâtres de lidocaine 5 % ou de capsaicine 8 %, pour le traitement de la douleur neuropathique localisee en premiere intention. Bien qu’il existe un besoin de developper davantage d’essais controles randomises pour ces deux topiques, les travaux publies demontrent un tres bon rapport benefice/risque, une bonne securite d’emploi et une efficacite a long terme. Les avantages majeurs de ces deux traitements sont leur niveau d’efficacite et le faible risque d’effets indesirables tels que confusion, somnolence, vertiges et constipation souvent associes aux traitements systemiques de la douleur neuropathique. Ils peuvent egalement etre utilises en association avec d’autres traitements et analgesiques sans interactions medicamenteuses majeures.

Abstract

International audience; Purpose: Patient education constitutes a relevant strategy to improve pain management. In the field of therapeutic patient education (TPE), we aimed 1) to assess pain impact in cancer patients, 2) to identify patients' educative needs in pain management, and 3) to refine research criteria for its future evaluation. Patients and methods: Pain intensity, relief and interference were assessed in 75 cancer patients with unbalanced background pain. Self-assessment questionnaire evaluated i) patients' pain management and ii) their knowledge and needs in TPE. Results: Most patients experienced pain for more than 6 months and 41.6% reported adequate pain relief. Understanding pain and pain management were major patients' preferences (>58%). Most patients declared they knew their pain treatments, but fewer than half of them were able to name them. However, education concerning pain treatment was considered as essential in

Abstract

International audience; Notre objectif était de faire un état des lieux des connaissances sur la prise en charge médicamenteuse des douleurs neuropathiques chez les patients avec cancer. Alors que le nombre de patients avec cancer ne cesse d’augmenter et que les douleurs sont très fréquentes lors de cancer, peu d’articles s’intéressent à la prise en charge médicamenteuse des douleurs, notamment neuropathiques, chez les patients ayant ou ayant eu un cancer. Cette revue systématique de l’ensemble des études publiées entre décembre 2012 et août 2018 devait permettre d’estimer l’ampleur du manque. Au total, 27 articles ont été identifiés sur la base d’une recherche systématique de la littérature (PubMed) et des connaissances des auteurs. Ces résultats confirment que peu de données ont été publiées. L’hétérogénéité des patients cancéreux, du cancer, et de la douleur expliquent en partie ce manque. Les recommandations, qui s’appuient sur des résultats obtenus auprès de patients généralement non cancéreux, suggèrent l’utilisation des antidépresseurs tricycliques et des antiépileptiques ; les traitements locaux sont appréciés pour leur bonne tolérance systémique. Des études incluant un plus grand nombre d’individus et tenant compte de l’étiologie de la douleur neuropathique, de ses caractéristiques (neuropathiques ou mixtes), et de la place de l’individu dans le parcours de soins (patient devant être traité, patient sous traitement anticancéreux, survivant récent ou ancien, ou patient en phase terminale) sont nécessaires pour pouvoir améliorer la connaissance. Les résultats de cette analyse de la littérature peuvent guider les chercheurs pour leurs futures études.

Abstract

International audience; Objective: Existing staging models have not been fully validated. Thus, after classifying patients with schizophrenia according to the staging model proposed by McGorry et al. (2010), we explored the validity of this staging model and its stability after one-year of follow-up.Method: Using unsupervised machine-learning algorithm, we classified 770 outpatients into 5 clinical stages, the highest being the most severe. Analyses of (co)variance were performed to compare each stage in regard to socio-demographics factors, clinical characteristics, co-morbidities, ongoing treatment and neuropsychological profiles.Results: The precision of clinical staging can be improved by sub-dividing intermediate stages (II and III). Clinical validators of class IV include the presence of concomitant major depressive episode (42.6% in stage IV versus 3.4% in stage IIa), more severe cognitive profile, lower adherence to medication and prescription of >3 psychotropic medications. Follow-up at one-year showed good stability of each stage.Conclusion: Clinical staging in schizophrenia could be improved by adding clinical elements such as mood symptoms and cognition to severity, relapses and global functioning. In terms of therapeutic strategies, attention needs to be paid on the factors associated with the more stages of schizophrenia such as treatment of comorbid depression, reduction of the number of concomitant psychotropic medications, improvement of treatment adherence, and prescription of cognitive remediation.

Abstract

International audience; Background: Communication between general practitioners (GPs) and hospitals is a weakness of French health system. Unfortunately, hospital discharge documents, a keypoint for healthcare safety and efficiency, tend to be poorly defined. This study aimed to propose standardised and reasoned models of the discharge letter (DL) and the discharge summary (DS) responding to the GPs’s requirements. Methods : A Delphi methodology was conducted from the findings of a systematic review of the international literature. We recruited 28 experts including producers (hospitalists and interns), recipients (GPs, member of the medical information department, patient representative), and other professionals using the discharge documents (referent of regional health administration for inspection and control, jurist, pharmacists).Results: Consensus was reached after two rounds of consultation. According to the experts the DS should be available for the last hospitalization day but it was rarely possible. Therefore the DL became the most important document for the GPs. Eventually two standardised and reasoned models were submited to the experts for validation. Models were considered as pertinent by the experts and perceived as an enhancement for several key points such as writing and reading speed, communication between hospitals and community practitioners or security of healthcare.Conclusion: The DL and DS models will be tested in lead hospitals by an impact study.; Introduction : Le lien ville-hôpital est une faiblesse de notre système de santé. Les documents de sortie d’hospitalisation qui sont l’élément-clé pour la sécurité et l’efficience des soins sont pourtant peu codifiés. L’objectif de ce travail était d’élaborer un modèle standardisé et raisonné de la lettre de liaison (LL) et du compte-rendu d’hospitalisation (CRH) en portant un soin particulier aux attentes des médecins généralistes ambulatoires (MG). Méthode : La méthode Delphi a été appliquée sur la base d’une revue systématique de la littérature publiée précédemment. Vingt-huit experts dont les rédacteurs (praticiens hospitaliers, internes), les destinataires (MG, représentant des patients, responsable de Département d’Information Médicale), mais aussi d’autres professionnels intervenant autour des documents de sortie (délégué ARS chargé de l’inspection et contrôle, juriste, pharmaciens) ont été recrutés.Résultats : Deux tours de consultation ont été nécessaires à l’obtention d’un consensus. Dans l’idéal le CRH devait être transmis au MG le jour de la sortie du patient mais ce n’était que rarement possible. Dans une logique de sortie à deux documents, la LL devenait l’élément central pour les MG. A l’issue des consultations, des documents standardisés et raisonnés ont été formulés. Ils ont été présentés et validés par les experts qui les ont jugés pertinents, pouvant améliorer la vitesse de rédaction et de lecture, la communication entre médecins hospitaliers et ambulatoires et la sécurité des soins en sortie d’hospitalisation.Conclusion : Les modèles LL et CRH proposés seront implantés dans des centres hospitaliers pilotes dans le cadre d’une étude d’impact.

Abstract

Pain, one of the most feared symptoms for patients with cancer, remains insufficiently alleviated and impairs quality of life. Therapeutic patient education (TPE) is a relevant approach to this problem while allowing patients to develop skills to better manage their pain. In the "Basse-Normandie" French region, the management of pain relies on two organized networks, thus allowing proximity and accessibility for all concerned. In this context, our team has begun a broad five-step research program that is part of a regional health policy: (1) training in TPE of 10 doctor/nurse pairs; (2) identification of educational expectations of patients and their relatives in the field of cancer pain; (3) design and optimization of a TPE program dedicated to cancer pain; (4) regional pilot study aiming to assess the feasibility, quality and transferability of the program; (5) evaluation of the TPE program by interventional comparative randomization at the national level. This article aims to present the program which originality and strengths are based on collaborative work between health stakeholders. Objectives, methodology and expected results of the research phase (stages 2, 4, 5) are notably developed. The main expected outcomes are to prove the effectiveness of the program in improving the knowledge and skills of patients in the field of pain cancer in order to promote their adherence to treatment and, consequently, to enable them to better manage it. The long-term objective is to disseminate the educational approach by modifying practices that provide a mutual benefit for caregivers and patients., La douleur est l'un des symptômes les plus redoutés par les patients atteints de cancer. Elle reste insuffisamment soulagée et retentit sur leur qualité de vie. L'éducation thérapeutique du patient (ETP) constitue une approche pertinente pour répondre à cette problématique en permettant au patient de développer des compétences pour mieux gérer sa douleur. En Basse-Normandie, la prise en charge de la douleur liée au cancer s'appuie notamment sur deux réseaux structurés permettant ainsi la proximité et l'accessibilité pour tous. Dans ce contexte, nous avons construit un large programme de recherche s'inscrivant dans une politique régionale de santé, en cinq étapes : (1) formation à l'ETP de 10 binômes médecins/infirmières ; (2) identification des attentes éducatives des patients et des proches dans le domaine de la douleur ; (3) conception et optimisation d'un programme ETP ; (4) étude pilote régionale permettant d'évaluer la faisabilité, la qualité et la transférabilité du programme ; (5) évaluation du programme d'ETP par randomisation comparative interventionnelle menée à l'échelle nationale. L'objectif de cet article est de présenter la stratégie de notre approche et plus particulièrement les objectifs, la méthodologie et les résultats attendus des phases de recherche (étapes 2, 4, 5). À l'issue des cinq étapes, notre ambition est de démontrer l'efficacité du programme éducatif pour améliorer les compétences des patients leur permettant de mieux gérer leur douleur. La finalité à long terme est de diffuser la démarche éducative avec une modification des pratiques apportant un bénéfice réciproque pour les soignants et les patients.

Abstract

Trace elements and minerals influence the pathogenesis of obesity and diabetes and their complications, mainly through their involvement in peroxidation and inflammation. On the other hand, peroxidation and inflammation are liable to alter themetabolismof these nutrients, which should be taken into account when assessing their status. Milk products are capable of preventing iron-induced peroxidation and the metabolic complications of the diseases. Calcium, especially cow milk calcium, seems to be capable of preventing the development of adipocytes and obesity complications (inflammation, hypertension, and insulin resistance), mainly through interactions with vitamin D metabolism. These observations give an opportunity to influence the course of obesity by dietetic advices.

10. LIBELULE: a randomized phase III study to evaluate the clinical relevance of early liquid biopsy in patients with suspicious metastatic lung cancer.

Purpose: Genomic profiling is a major component for first-line treatment decisions in patients with non-small cell lung cancer (NSCLC) and the timeliness of biomarker testing is essential to improve time to treatment initiation (TTI) or avoid inappropriate treatment., Patients and Methods: The phase III LIBELULE trial (NCT03721120) included patients with radiological suspicion of advanced lung cancer. They were randomized (1:1), the control arm receiving diagnostic procedures according to each center's practice and the liquid biopsy arm with additional testing performed at the first visit using the InVisionFirst®-Lung assay. Treatment initiation and type were defined according to ESMO guidelines. Primary endpoint was the time from randomization to initiation of appropriate treatment based on informative genomic and pathological results in the intention-to-treat population., Results: 319 patients were enrolled (LB: 161; control: 158). Median age was 68 years, 28.8% were non-smokers, 18.1% had PS≥2 and 56.7% had adenocarcinoma. In LB arm, 81% of patients had circulating tumor DNA findings. The mean TTI was not significantly reduced (LB: 29.0 days (d); control 34d (p=0.26)). Sensitivity analyses showed a shorter TTI in patients from the LB arm who received systemic treatment (LB: 29.1d; control: 38.8d, p=0.01), in patients with advanced non-squamous NSCLC (LB: 29.5d; control: 40.3d, p=0.01), and in patients with first-line targetable alterations (LB: 21d; control 37.4d) (p=0.004). Time to contributory genomic results was significantly reduced (LB: 17.9d; control 25.6d, p<0.001)., Conclusion: Early liquid biopsy testing did not significantly shorten the TTI in unselected patients referred for suspected advanced lung cancer. Abstract

Chronic postsurgical neuropathic pain (CPSNP) is frequent. While prevalence varies considerably according to type of operation and means of evaluation, it can reach 37% following breast surgery. Identification of risk factors related to the procedure and to the patient and taking into account the development of new, minimally invasive surgical techniques is increasingly nerve-sparing and reduces the likelihood of injury. CPSNP diagnosis in daily practice is facilitated by simple and quickly usable tools such as the NP4 4-question test. Management is based on pharmacological (analgesics, antiepileptics, antidepressants, local anesthetics) and non-pharmacological (kinesitherapy, neurostimulation, psychotherapy) approaches. Abstract

The objective of the present systematic literature review was to provide an update on medical treatment of neuropathic pain in cancer patients. The number of cancer patients is steadily increasing. Pain is frequent in cancer patients. Few studies have focused on medical treatment of pain, and especially of neuropathic pain, in current or former cancer patients. The present systematic review of all studies published between December 2012 and August 2018 was intended to estimate the scale of this lack. In all, 27 articles were identified on a systematic PubMed search and from the authors' personal knowledge, confirming that scant data have been published. The heterogeneity of cancer patients, of cancer, and of pain go some way toward explaining this scarcity. Guidelines, founded mainly on results from non-cancer patients, recommend tricyclic antidepressants and antiepileptic drugs; local treatments have the advantage of good systemic tolerance. Larger-scale studies taking account of the etiology of neuropathic pain, its characteristics (strictly neuropathic or mixed) and patient characteristics (awaiting treatment, under treatment, recent or non-recent survivor, or in terminal phase) along the care pathway are needed to improve knowledge. Abstract

This paper reviewed the 2002 guidelines established by the National Federation of Cancer Centres. A group of experts nominated by the 3 French Societies involved in the treatment of cancer pain (AFSOS, SFAP, SFETD), established new guidelines ratios for morphine switching and/or changing of route of administration, in patients for whom either pain was not adequatly managed or adverse effects were unbearable. After a rapid reminder of the pharmacokinetics and metabolism properties of morphine, experts explained why the theory of opioid rotation (oxycodone, hydromorphone, fentanyl, methadone, tapentadol) using fixed equianalgesic ratios is not any more appropriate for a secure clinical practice. In the light of recent publications enhancing our knowledge on the efficacy of new drug switching ratios and for changing the route of administration of morphine, the group of experts recommended to use reconsidered switching ratios favoring security upon efficacy, to minimize overdosing and adverse effects. Consequently, after the new conversion ratio (using slow release opioids) was applied, a second titration should be done by means of normal release rescue formulations for breakthrough pain episodes. Abstract

Aim: Cetuximab is an anti-epidermal growth factor receptor antibody used for the treatment of metastatic colorectal cancer and head and neck cancer. Hypersensitivity reactions (HSRs) are associated with cetuximab use. The aim of the study was to evaluate the utility of anti-cetuximab immunoglobulin E (IgE) detection in order to identify patients at risk of HSR to cetuximab., Methods: We included patients ready to receive a first cetuximab infusion in a prospective cohort carried out at nine French centres. Pretreatment anti-cetuximab IgE levels were measured. We compared the proportion of severe HSRs in the low anti-cetuximab IgE levels (≤29 IgE arbitrary units) subgroup with that in a historical cohort of 213 patients extracted from a previous study., Results: Of the 301 assessable patients (mean age: 60.9 ± 9.3 years, head-and-neck cancer: 77%), 66 patients (22%) had high anti-cetuximab IgE levels, and 247 patients received cetuximab (including 38 with high anti-cetuximab levels). Severe HSRs occurred in eight patients (five grade 3 and three grade 4). The proportion of severe HSRs was lower in the low anti-cetuximab IgE levels subgroup vs. the historical cohort (3/209 [1.4%] vs. 11/213 [5.2%], odds ratio, 0.27, 95% confidence interval, 0.07-0.97), and higher in high vs. low anti-cetuximab IgE levels subgroup (5/38 [13.2%] vs. 3/209 [1.4%]; odds ratio, 10.4, 95% confidence interval, 2.4-45.6). Patients with severe HSRs had higher anti-cetuximab IgE levels than patients without reaction (median, 45 vs. 2 IgE arbitrary units, P = 0.006)., Conclusions: Detection of pretreatment anti-cetuximab IgE is feasible and helpful to identify patients at risk of severe cetuximab-induced HSRs., (© 2016 The British Pharmacological Society.)

Abstract

There is a lack of numeric data for the mechanical characterization of spine muscles, especially in vivo data. The multifidus muscle is a major muscle for the stabilization of the spine and may be involved in the pathogenesis of chronic low back pain (LBP). Supersonic shear wave elastography (SWE) has not yet been used on back muscles. The purpose of this prospective study is to assess the feasibility of ultrasound SWE to measure the elastic modulus of lumbar multifidus muscle in a passive stretching posture and at rest with a repeatable and reproducible method. A total of 10 asymptotic subjects (aged 25.5 ± 2.2 years) participated, 4 females and 6 males. Three operators performed 6 measurements for each of the 2 postures on the right multifidus muscle at vertebral levels L2-L3 and L4-L5. Repeatability and reproducibility have been assessed according to ISO 5725 standard. Intra-class correlation coefficients (ICC) for intra- and inter-observer reliability were rated as both excellent [ICC=0.99 and ICC=0.95, respectively]. Reproducibility was 11% at L2-L3 level and 19% at L4-L5. In the passive stretching posture, shear modulus was significantly higher than at rest (µ < 0.05). This preliminary work enabled to validate the feasibility of measuring the shear modulus of the multifidus muscle with SWE. Abstract

Objectives: Know the impact of the unplanned deliveries in a town of medium size, the characteristics of these women and maternal and neonatal risks., Materials and Methods: This was a retrospective study conducted between January 2002 and December 2009. Unexpected delivery was defined as any delivery taking place outside of a non-elective way maternity. Each unexpected delivery was matched at nearest delivery of equivalent term, at the CHU maternity, with an onset of spontaneous labour., Results: Ninety-four women gave birth unexpectedly for a total of 48,721 births (incidence of 0.19%). There was a significant difference between cases and controls for parity (1.8 versus 0.9), the lack of follow-up of pregnancy (21.3% versus 1.1%), tobacco (57.4% versus 25.5%), the socio-economic level, the type of feeding (artificial: 61.7% versus 30.6%), the home-hospital distance and obstetric follow-up. We found a significant increase in perinatal mortality (6.4% versus 1%) and stay in Neonatal ICU (19.1% versus 9.2%). The main neonatal morbidity was hypothermia., Conclusion: It is difficult to target a population at risk because the type of these women is non-specific. Prevention of unplanned deliveries and their morbidities through information of patients on the grounds of urgent consultation and support of the newborn to limit hypothermia., (Copyright © 2015 Elsevier Masson SAS. Abstract

The aim of this study was to assess the relationships of fatness and fitness with metabolic risk factors, including liver transaminases and inflammation in obese youth, taking in account gender, age, and pubertal stage. 241 children were studied (135 girls), age 11.9 +/- 2.2 years (x +/- SD), Body Mass Index z score 5.4 +/- 2.7.
