Amaç: Agomelatin, melatonin reseptör (MT1 ve MT2) agonisti ve serotonin reseptör (5-HT2C) antagonisti olan antidepresan bir ilaçtır. Artan kanıtlar, agomelatinin nöroprotektif ve nöromodülatör etkiye sahip olduğunu göstermektedir. Bu çalışmada skopolamin indüklü bilişsel yetmezlik oluşturulan sıçanlarda agomelatinin potansiyel etkileri araştırılmıştır. Materyal ve Metot: Erişkin erkek sıçanlara 21 gün süreyle skopolamin (1 mg/kg) ve agomelatin (40 mg/kg) uygulandı. İlaç uygulamalarını takiben sıçanlar bilişsel davranışların değerlendirilebilmesi amacıyla yeni nesne tanıma (YNT) ve Morris su labirenti (MSL) testine tabi tutuldu. İlave olarak, beyin nörokimyasal analizleri için hipokampus ve prefrontal kortekste beyin-türevi nörotrofik faktör (BDNF) ve asetilkolin (ACh) düzeyleri değerlendirildi. Bulgular: Skopolamin hem uzamsal hafızayı hem de ayırt etme indeksini önemli ölçüde azalttı (p0,05). Ayrıca agomelatin, skopolamin grubuna kıyasla hem hipokampusta hem de prefrontal kortekste BDNF düzeylerini önemli ölçüde arttırdı (sırasıyla p0,05). Sonuç: Birlikte ele alındığında, bu sonuçlar agomelatinin skopolamin kaynaklı hafıza yetmezliğinin hafifletilmesinde belirgin rol oynadığını göstermiştir. Bu nedenle, agomelatinin bilişsel yetmezliğin önlenmesinde potansiyel bir ajan olabileceğini öne sürüyoruz Objective: The antidepressant agomelatine agent is a melatonin receptor (MT1 and MT2) agonist and a serotonin receptor (5-HT2C) antagonist. Increasing evidence shows that agomelatine has neuroprotective and neuromodulatory effects. In this study, the potential effects of agomelatine in rats with scopolamine-induced cognitive impairment were investigated. Materials and Methods: Adult male rats were administered scopolamine (1 mg/kg) and agomelatine (40 mg/kg) for 21 days. After drug administration, rats were subjected to new object recognition (NOR) and Morris water maze (MWM) tests in order to evaluate cognitive behaviors. In addition, brain-derived neurotrophic factor (BDNF) and acetylcholine (ACh) levels in the hippocampus and prefrontal cortex were evaluated. Results: Scopolamine significantly decreased both spatial memory and discrimination index (p0.05). In addition, agomelatine significantly increased BDNF levels in both hippocampus and prefrontal cortex compared to the scopolamine group (p0.05). Conclusion: Taken together, these results demonstrated that agomelatine plays a important role in alleviating scopolamine-induced memory impairment. Therefore, we suggest that agomelatine may be a potential agent in the prevention of cognitive impairment.

Yüksek Lisans Tezi, Bu çalışmanın amacı, in vitro mesane kasılma yanıtları üzerinde agomelatin, fluoksetin ve sertralinin olası etkilerinin belirlenerek, bu etkilerin birbirleri ile karşılaştırılması ve bu antidepresanların aşırı aktif mesane sendromuna katkılarının araştırılmasıdır. Deneysel çalışmalar Necmettin Erbakan Üniversitesi KONÜDAM Deneysel Tıp Araştırma ve Uygulama Merkezi ile Necmettin Erbakan Üniversitesi Meram Tıp Fakültesi Fizyoloji Anabilim Dalı Laboratuvarında gerçekleştirildi. Araştırmamızda erişkin (300 - 350 g ağırlığında) erkek Wistar Albino cinsi sıçanlar kullanıldı. Hayvanlara hafif eter anestezisi altında servikal dislokasyon uygulandı. Mesane hızlı bir şekilde ayrıldı ve Krebs solüsyonu içerisine alındı. Doku ve kan artıklarının temizlenmesinden sonra, mesaneden kesilen 3 - 4 milimetre uzunluğundaki şeritler sıcaklığı 37˚C olan ve sürekli gazlandırılan (%95 O2 ve %5 CO2), içinde krebs solüsyonu olan izole organ banyosuna yerleştirildi ve gerim 1 g olarak ayarlandı. Dokular asıldıktan sonra 15 dakikalık periyodlarla 1 saat yıkandı. İzometrik kontraksiyonlar 10-5 M ACh ile indüklenip, agomelatin (10-9 M ve 10-4 M), fluoksetin(10-8 M ve 10-3 M), sertralin (10-8 M ve 10-3 M) 10'ar dk arayla, organ banyosuna ilave edildi. Kontraksiyonlar frekans ve genlik parametreleri olarak izole organ banyosu sisteminde kayıt altına alındı. Agomelatin, fluoksetin ve sertralinin farklı dozlarının ACh 10-5 M ile indüklenen in vitro rat mesane düz kas kontraktilitesini inhibe ettiği belirlendi. Agomelatinde 10-8 M ve 10-7 M'dan itibaren belirgin bir inhibisyon gözlendi. Lisansüstü tezlerin elektronik ortamda toplanması, düzenlenmesi ve erişime açılması konusuna ilişkin yök'ün 18.06.2018 tarihli yönergesine istinaden artık bu tarihten sonra gizlilik şartı aranmayan bütün tezler erişime açılacaktır. Amaç: Bu çalışmada ki amacımız ketamin ile oluşturduğumuz deneysel şizofreni modelinde agomelatin'in bilişsel fonksiyonlar üzerine etkileri araştırmaktır. Gereç ve Yöntem: Çalışmamızda erkek fareler dört gruba (n:8) ayrıldı. Kontrol grubuna salin enjeksiyonu 0,1 ml hacminde 14 gün boyunca intraperitoneal (i.p.) olarak uygulandı. Ketamin (KET) grubuna şizofreni oluşturmak için 14 gün boyunca 30 mg/kg/gün dozunda ketamin i.p uygulandı. Ketamin+Agomelatin (KET+AGO) grubuna ise 14 gün 30 mg/kg dozunda ketamin (i.p.) verilirken, 5-14. günler arası 50 mg/kg/gün dozunda (0,1 ml) agomelatin oral gavaj şeklinde verildi. Agomelatin (AGO) grubuna 50 mg/kg/gün dozunda (0,1 ml) agomelatin oral olarak uygulandı. Çalışmanın 14 ve 15. günlerinde açık alan, zorunlu yüzme ve yabancı obje tanıma testi yapıldı. Beyin doku örneklerinde beyin kaynaklı nörotrofik faktör (BDNF), glutatyon peroksidaz (GPx) ve süperoksit dismutaz (SOD) değerleri analiz edildi. Bulgular: Açık alan testinde; KET grubunun kontrol grubuna göre ortalama hız ve mesafe değerlerinin anlamlı olarak yükseldiği, KET+AGO ve AGO gruplarında ise KET grubu ile karşılaştırıldığında anlamlı olarak azaldığı belirlenmiştir. Yeni obje tanıma testi sonuçlarında istatistiksel olarak anlamsız bulunmuştur. Zorunlu yüzme testinde; KET+AGO grubunda hareketsiz olarak geçirilen sürenin KET grubuna göre anlamlı bir artış gösterdiği görülmüştür. BDNF değerlerine bakıldığında KET grubu kontrol grubuna göre anlamlı olarak azaldığı, KET+AGO ve AGO gruplarında ise BDNF düzeyleri KET uygulanan gruba göre anlamlı olarak arttığı gözlenmiştir. Fakat GPx ve SOD değerleri açısından gruplar arasında anlamlı farklılık bulunmamıştır. Sonuç: Sonuç olarak şizofreni hastaları için tedavide yaşanan gelişmelere rağmen halen önemli bir sorun olan bilişsel ve motor fonksiyonlardaki bozuklukları düzeltmede agomelatin özellikle yan etki profilinin düşük olması sebebiyle tercih edilebilir. Aim: In this study, the effects of agomelatin on cognitive functions in ketamine-induced schizophrenia model were investigated. Material and Method: In our study, male rats were divided into four groups. Saline injection to control group was administered intraperitoneally (i.p.) in 0.1 ml volume for 14 days. The ketamine (KET) group received 30 mg/kg ketamine i.p for 14 days to induce schizophrenia. Ketamine+Agomelatin (KET+AGO) group received ketamine (i.p.) at a dose of 30 mg/kg/day for 14 days. Between the days of 50 mg/kg/day dose agomelatin in the form oral gavage and agomelatin group was administered orally at dose of 50 mg/kg/day. On the 14th and 15th days of the study, open area, compulsory swimming and foreign object recognition test were performed. However, there was no significant difference between the groups in terms of GPx and SOD values. Results: In the open field test; the mean velocity and distance values of the KET group increased significantly compared to the KNT group and decreased significantly in the KET+AGO and AGO groups compared to KET group. New object recognition test results were found to be statistically insignificant. In compulsory swimming test; when the KET group and the KNT group were compared, the time spent immobile increased significantly. When BDNF values were examined, it was observed that KET group decreased significantly compared KNT group and BDNF levels were significantly higher in KET+AGO and AGO groups compared to KET group. However, there was no significant difference between the groups in terms of GPx and SOD values. Conclusion: In conclusion, despite improvements in treatment for schizophrenia patients, agomelatin may be preferred for correcting cognitive and motor dysfunction which is still an important problem because of its low side effect profile.

In long-term stress exposure, excess glucocorticoids disturb the balance of monoamine neurotransmitters leading to mood disorders and memory impairment. Venlafaxine and Agomelatin are currently used to treat these disorders. Voluntary exercise also has beneficial effects on mental health. In this work, we analyzed 1. the time-dependent changes in stress-induced mood disorders, 2. the modulating effect of voluntary exercise on the hypothalamic pituitary adrenal axis, 3. the effectiveness of Agomelatin, Venlafaxine and exercise to prevent stress-related behaviors and 4. the localization of MT1 and MT2 receptors in transgenic reporter mice. We demonstrate that stress caused physical, emotional and behavioral abnormalities in stressed rats. Pre-treatment with Agomelatin, Venlafaxine and exercise reduced the chronic stress-related behaviors and prevented anxiety, depression and memory deficits. The mapping of MT1 and MT2 receptors identified potential sites of action of Agomelatin.; L’excès de glucocorticoïdes lors d’un stress prolongé perturbe la neurotransmission monoaminergique et mène à des troubles de l’humeur et de la mémoire. La Venlafaxine (Vlx) et l’Agomelatine (Ago) sont utilisés pour traiter ces troubles. L’exercice physique volontaire est aussi bénéfique pour la santé mentale. Nous avons analysé 1. les changements de l’humeur induits par le stress en fonction du temps, 2. l’effet de l’exercice volontaire sur l’axe hypothalamo-pituitaire, 3. l’efficacité de l’Ago, de la Vlx et de l’exercice à prévenir les perturbations liées au stress et 4. la localisation des récepteurs MT1 et MT2 chez des souris rapportrices transgéniques. Nous démontrons que le stress induit des dérèglements physiques, émotionnels et comportementaux chez des rats stressés. Le prétraitement par l’Ago, la Vlx et l’exercice préviennent l’anxiété, la dépression et les déficits de mémoire. La cartographie des récepteurs MT1 et MT2 a identifié des sites d’action potentiels de l’Ago.

Doktora Tezi, Amaç: Bu tez çalışmasının amacı, in vitro mesane kasılmaları üzerine melatonin reseptörlerinin olası etkilerinin ve bu etkilerin meydana gelmesinde reseptör agonist ve antagonistinin rollerinin araştırılmasıdır. Yöntem: Deneysel çalışmalar, Necmettin Erbakan Üniversitesi KONÜDAM Deneysel Tıp Araştırma ve Uygulama Merkezi ile Necmettin Erbakan Üniversitesi Meram Tıp Fakültesi Fizyoloji Anabilim Dalı Laboratuvarı'nda gerçekleştirildi. Çalışmada Wistar cinsi 5-6 aylık, 200–250 g arası 24 adet dişi sıçan kullanıldı. Eter sedasyonunu takiben servikal dislokasyon yöntemi ile sıçanlar ötenazi edildi. Abdomen medyan hattan açılarak mesane çıkarıldı ve Krebs solüsyonu içine alındı. Mesane boyun kısmından apex yönünde longitudinal bir kesi ile açılarak mesaneden vertikal yönde 2X10 mm ebadında iki kas şeridi hazırlandı. Şeritler izole organ banyosundaki cam hazneler içerisindeki düzeneğe 1 g gerim uygulanarak yerleştirildi. Kas şeritlerine melatonin, melatonin reseptör agonist ve antagonisti ve bunların çözücüleri kümülatif olarak uygulandı. 45 dakikalık bir uyum periyodunu takiben spontan kasılma gösteren bütün mesane şeritlerine 10-5 M asetilkolin (ACh) uygulanarak kasılmalar indüklendi. Kümülatif olarak agomelatin (10-8 M-10-3 M), melatonin (0.01 mM, 0.1 mM, 1 mM, 2.5 mM) ve melatonin 2 reseptör antagonisti 4PPDOT (0.01 mM, 0.1 mM) uygulamaları yapıldı ve genlik üzerinde oluşan etkiler kayıt altına alındı. Rats were euthanized by cervical dislocation after light ether sedation. The abdomen was opened by median line and the bladder was removed and taken into the Krebs solution. The bladder was opened with a longitudinal incision in the direction of the apex from the neck to prepare two muscular strips of 2x10 mm in the vertical direction. The strips were placed by applying 1 g of tension to the assembly in the isolated organ bath. Melatonin, melatonin receptor agonist and antagonist and their solubilizers were cumulatively applied to the muscles trips in organ bath. Following a 45-minute equal ibrati on period, tonic contractions were induced by application of acethylcholin (ACh) at 10-5 M concentration. Agomelatin (10-8 M-10-3 M), melatonin (0.01 mM, 0.1 mM, 1 mM, 2.5 mM) and 4PPDOT (a selective melatonin receptor 2 antagonist, 0.01 mM, 0.1 mM) were applied and the effects on contraction amplitudes were recorded. Diabetes mellitus (DM), kan şekeri değerlerinde yükselme ile seyreden metabolik bir bozukluktur. Hipergliseminin uygun tedavi ile kontrol altına alınamadığı durumlarda; mikrovasküler, makrovasküler ve nöropatik komplikasyonlar görülmektedir. Diyabetik nöropati, somatik veya otonom sinirlerin hasarına bağlı olarak nöropatik ağrıya neden olmaktadır.Nöropatik ağrının progresyonunu yavaşlatan en iyi yöntem glisemik kontrol olmakla birlikte, semptomatik tedavide antidepresanlar, antikonvülzanlar veya opioidler gibi ilaçlar da sıklıkla kullanılmaktadır. Agomelatin, hem melatoninerjik hem de seratonerjik sistem üzerinden etkili, tedavi uyumu yüksek, yan etkileri düşük ve nöroplastisiteyi düzenleyeci yeni bir antidepresandır. Bu tez çalışmasında, agomelatin'in diyabetik sıçanlardaki nöropatik ağrı düzeyleri üzerindeki davranışsal etkinliğinin ve duyu nöronları üzerindeki etkilerinin morfometrik yöntemlerle araştırılması amaçlanmıştır. Deneylerde kullanılan erkek Spraque-Dawley ırkı erişkin sıçanlar; Konrol, DM, DM+Agomelatin (40 mg/kg) ve DM+Agomelatin (80 mg/kg) olmak üzere 4 gruba ayrılmıştır. İlaç tedavisi, STZ enjeksiyonu ile diyabet oluşturulduktan sonra periferik nöropati gelişimi için 4 hafta daha beklendikten sonra başlatılmış ve 14 gün süreyle oral gavaj yoluyla devam ettirilmiştir. Sıçanların (n=8/her grup) aktivite kafesindeki yatay ve dikey hareket sayıları ile mekanik ve termal ağrılı uyaranlara karşı verdikleri yanıt süreleri 4., 5. ve 6. haftalarda tekrarlanan davranış testleri ile değerlendirilmiştir. Daha sonra kardiyak perfüzyon ile sakrifiye edilen sıçanların 4. ve 5. lumbal seviyelerdeki arka kök ganglionları diseke edilmiştir. Bu dokular araldit bloklara gömüldükten sonra alınan ince kesitler toluidin mavisiyle boyanmıştır. Kesitlerden elde edilen mikroskobik görüntüler üzerinden, ganglion spinale'deki nöronların ortalama soma çapları ve lif çapları morfometrik yöntemler kullanılarak ölçülmüştür.STZ ile indüklenen diyabetik sıçanların lokomotor aktivitelerinde, kuyruk çekme ve kuyruk sıkıştırma testlerindeki ağrılı uyaranlara verdikleri yanıt sürelerinde kontrol gruplarındakilere oranla anlamlı düzeylerde azalma olduğu tespit edilmiştir. Morfometrik analizlerde de diyabetli sıçanların ortalama soma çapı ve lif uzunluğu değerlerinin kontrol gruplarındakilere göre anlamlı düzeyde düşük olduğu bulunmuştur. Agomelatin ile tedavi edilen gruplarda, ilacın özellikle düşük dozdaki uygulamaları, davranış bozukluklarının ve yapısal değişikliklerin geri döndürülmesinde daha etkili olmuştur.Bu sonuçlar, diyabetik nöropatinin tedavisinde düşük doz agomelatin uygulamasının ganglion spinale'deki duyu nöronlarında yapısal değişiklikler oluşturarak hiperaljezik yanıtın önlenmesinde etkili olduğunu göstermektedir. In cases of hyperglycemia cannot be controlled with proper treatment; microvascular, macrovascular and neurological complications are seen. Diabetic neuropathy causes neuropathic pain by damaging somatic or autonomic nerves.Although the best method of slowing the progression of neuropathic pain is the glycemic control; medications such as antidepressants, anticonvulsants or opioids are often used in the symptomatic treatment. Agomelatine is a new antidepressant modifying neuroplasticity by displaying its effects on both melatoninergic and serotonergic systems, and having high treatment compliance and low side effects. In this thesis study, it was aimed to investigate the effects of agomelatine on the behavioral effectiveness of neuropathic pain levels and its effects on sensory neurons by using morphometric methods. Male Sprague-Dawley rats used in the experiments were divided into 4 groups: Control, DM, DM + Agomelatine (40 mg / kg) and DM + Agomelatine (80 mg / kg). Drug treatment was started 4 weeks after STZ injection, in order to wait for the development of peripheral neuropathy, and was continued by oral gavage for 14 days. The number of horizontal and vertical movements of rats (n = 8 / each group) in the activity cage and their responses to mechanical and thermal noxious stimuli were assessed by behavioral tests repeated at 4th, 5th and 6th weeks. Then, animals were sacrificed by cardiac perfusion and the dorsal root ganglia of the 4th and 5th lumbar levels were dissected. After burial of these tissues into araldite blocks thin sections were taken and stained with toluidine blue. By using the microscopic cross section images obtained from these sections, the mean soma and fiber diameters of neurons in the spinal ganglia were measured by using morphometric methods.Locomotor activity and latency to painful stimuli in the tail clip and tail flick tests were significantly less in STZ-induced diabetic rats in comparison to control group. Morphometric analysis of the mean soma and fiber diameters of diabetic rats was significantly lower than those of controls. In groups treated with agomelatine, application of the drug at lower doses was more effective in reversing the behavioral disorders and structural changes. These results show that low-dose agomelatine application is effective in preventing hyperalgesic response in the treatment of diabetic neuropathy by causing structural changes in the sensory neurons of the spinal ganglia.Key words: Diabetic neuropathy, Agomelatine, Morphometry, Spinal ganglion, Tail clip, Tail flick, Activity meter. 85

Bu çalışma, köken itibariyle seçkinci bir siyasi gelenekten gelen ve tek parti dönemi boyunca seçkinci geleneğini devam ettiren Cumhuriyet Halk Partisi'nin, çok partili hayata geçiş ve muhalefet dönemlerinde ideolojisindeki ve politikalarındaki esneme ve ılımlılaşmayı incelemek amacıyla yapılmıştır.Tez üç bölüme ayrılmıştır. Birinci Bölüm'de, Türk siyasal hayatında köşe taşı konumunda olan ve kuruluşundan günümüze kadar siyasi, iktisadi ve toplumsal alanda izleri görülen bir parti olan Cumhuriyet Halk Partisi'nin kökeni, kuruluşu, tek parti dönemi olarak tabir edilen dönemdeki icraatları ve ideolojisi incelenmiştir. İkinci Bölüm'de çok partili hayata geçiş aşamasında Cumhuriyet Halk Partisi'nin rolü ve etkileri ve çok partili siyasal sürece geçiş aşamasında partinin iç dinamiklerindeki değişim ve siyasi ve iktisadi alanlarda bu dönem itibariyle daha esnek politikalara geçiş yapması incelenmiştir. The study has divided into three sections: In the first section, CHP, which has had cornerstone position in Turkish political life since its establishment, will be analyzed from political, economic and societal aspects of its rulership from the beginning of 1920s to 1950. In the second section, CHP's effects of transition to multiparty system in the political process and the party's internal dynamics in the transition phase will be examined. In the third section, an election result of 1950, which made CHP an opposition party, will be handled. The 1950 election results caused a chaotic atmosphere in the party because of the party's intenal dynamics. Because the begininng of 1950s to 1955, CHP endeavoured to accept its new position as the oppostion party. During the single party era, while CHP had adopted in statist, secular and elitist policies, it began to change its political and economic stance towards multiparty system and economic liberalization by the effects of internal and external motives. Although CHP was trying to carry out political change under its control and leadership, the process of transition to multiparty system evolved something that was out of control. Bei 16-20% der Frauen kommt es zu einer Erstmanifestation einer Depression in der Perimenopause. In insgesamt 60% der Fälle ist eine Verschlechterung vorbestehender Depressions- Angstsymptome beschrieben. Weiterhin steigt das Risiko, eine Depression zu entwickeln, um das 14fache im Vergleich zu ca. 31 Jahren prämenopausaler Zeit. Auch steigt die Suizidrate bei Frauen zwischen dem 45. Und 64. Lebensjahr an. Insbesondere wird in 75% der betroffenen Frauen über signifikante Schlafstörungen berichtet, welche wiederum auf hot flashes in der ersten Nachthälfte zurückzuführen sind. Ebenso führt das zunehmende Östrogendefizit zu einer serotonergen Dysfunktion und damit wiederum zu Schlafstörungen. In der Folge kommt es bei den perimenopausalen Frauen zu einem circulus vitiosus aus Insomnie und Depressivität. In dieser Arbeit wurde die Prüfsubstanz Agomelatin bei 40 Frauen mit diagnostizierter Depression im Hinblick auf die Therapie depressiver und perimenopausaler körperlicher Beschwerden über einen Zeitraum von 6 Wochen mittels standardisierten Interviews untersucht. Bei dieser Untersuchung handelt es sich um eine offene, randomisierte, nicht-kontrollierte Pilotstudie. Pharmakokinetisch wirkt Agomelatin agonistisch auf die melatonergen MT1- und MT2-Rezeptoren sowie kompetitiv antagonistisch auf die serotonergen 5HT2c-Rezeptoren im Nucleus suprachiasmaticus. Durch diesen Angriffsort kann der Arzneistoff auf die circadianen Rhythmen modulierend bzw. resynchronisierend einwirken. Die thymoleptische Wirkung von Agomelatin lässt sich jedoch nicht nur der melatonergen Komponente oder dem Antagonismus am serotonergen 5HT2c-Rezeptor zuordnen, vielmehr ist ein Zusammenspiel beider Wirkungen hierfür verantwortlich. Ein Erklärungsansatz bildet die antagonistische Wirkung an den 5HT2c-Rezeptoren, welche zu einer Blockade der stimulierenden und daher am Abend unerwünschten Wirkung des Serotonins am Nucleus suprachiasmaticus führt und dadurch den melatonin-agonistischen Effekt verstärkt. Zudem vermittelt die 5HT2c-Rezeptor-Blockade eine indirekte Erhöhung von Noradrenalin und Dopamin im frontalen Cortex und sorgt so im Umkehrschluss für eine zzgl. thymoleptische Wirkung. Durch den Antagonismus am 5HT2c- Rezeptor wird außerdem der Tiefschlaf verbessert und somit folglich auch die Schlafqualität und die Tagesvigilanz. In diesem Rahmen stach Agomelatin mit seinem von Östrogen unabhängigem Rezeptorpotential besonders hervor, indem es sowohl depressionslösend als auch auf körperliche Beschwerden reduzierend wirkte. Über den 6wöchigen Zeitraum konnte sowohl eine signifikante Verbesserung des Schweregrades der Depressionsausprägung wie auch eine dramatische Linderung der körperlichen Symptome wie hot flashes, Leistungsminderung, Schlafstörungen, Irritabilität, sexuelle Dysfunktionen, Cephalgien, Gelenk- und Muskelbeschwerden nachgewiesen werden. Nach Rückgang der vegetativen Beschwerden war auch eine Regredienz der Depression festzustellen, und die Mehrzahl der Patientinnen remittierte insgesamt in ihrem Beschwerdebild. Am Ende der Studie gaben die Studienteilnehmerinnen an, sowohl affektiv wie auch körperlich frei von perimenopausalem Leiden zu sein., 16%-20% of all women happen to have a primal manifestation of a depression during their perimenopausal years. In 60% of all cases a deterioration of preexisting symptoms of a depression and anxiety is described. Furthermore the risk of developing a depression rises by 14 times compared to about 31 years of premenopausal time. Additionally the suicidal rate of females between the ages of 45 and 64 rises. Especially significant insomnia, caused by hot flashes during the first part of the night, can be detected at 75% of all women affected. The increasing deficit of estrogen also leads to serotonergic dysfunction and thus again to insomia. Finally a so called circulus vitae consiting of insomnia and depression is the consequence for perimenopausal women. This paper focused on 40 women taking the test piece Agomelatin. All 40 women had diagnosed perimenopausal depression. Agomelatin and the 40 women were examined with regard to the therapy of depressive discomfort as well as perimenopausal physical discomfort by using standardized interviews covering a six-week time span. This research was an open, randomized and not-controlled pilot study. From a pharmakokinetical point of view Agomelatin on the one hand agonistically affects the melatonergic MT1- and MT2-receptors and on the other hand competitively antagonistic the serotonergic 5HT2c-receptors placed at nucleus suprachiasmaticus. Through this docking area the drug can influence the circadian rhythms in a modulating re-synchronizing way. The thmoleptic effect of Agomelatin can not only attributed to the melatonergic component or the antagonism at the serotonergic 5HT2c-receptor- far more strongly an interaction of both effects can be made responsible. A possible explanation is the antagonistic effect happening at the 5HT2c-receptors. The 5HT2c-receptors lead to blockade of the stimulating- thus in the evening unwanted- effect of serotonin at the nucleus suprachiasmaticus and thereby increases the agonistic effect on melatonin. Furthermore the 5HT2c-receptor blocking leads to an increase of noradrenalin and dopamin in the frontal cortex. This finally ensures an additional thymoleptic effect. Due to the antagonism at the 5HT2c deep sleep is improved and thus finally also the quality of sleep as well as the daily vigilance. In this research Agomelatin, with its estrogen- independent potential of receptors, especially sticked out by being depression-relieving and physical pain-reducing. Over this six weeks time span it was possible to prove a significant improvement of the severity code of the characteristic of the depression on the one hand and a dramatic alleviation of the following symptoms on the other hand: hot flashes, loss of efficiency, insomnia, irritability, sexual dysfunctioning, cephalgia and joint- and muscle pain. After the reduction of vegetative discomfort a regredience of depression was detected and the majority of female patients remitted regarding their symptoms. At the end of the research all female patients declared that they were free of perimenopausal pain, affectively as well as physically.

Agomelatin'in akut ve subakut antinosiseptif etkinliği kuyruk sıkıştırma ve sıcak plaka testleri ile değerlendirilmiştir. Bu ilacın nöropatik ağrı üzerine olan etkisi ise mekanik ve termal uyaranların uygulandığı farklı deneysel modellerde incelenmiştir. Bu kapsamda, mekanik allodini ölçümleri için dinamik plantar aesthesiometer testi; mekanik hiperaljezi ölçümleri için Randal-Sellito testi; termal allodini ölçümleri için ılık plaka (38oC) testi ve termal hiperaljezi ölçümleri için de Hargreave's ve soğuk plaka (4oC) testleri uygulanmıştır. Elde edilen veriler agomelatin'in 40 ve 80 mg.kg-1 dozlarda akut uygulamasının, normoglisemik sıçanlarda opioid mekanizmaların aracılık ettiği güçlü bir antinosiseptif etkiye neden olduğunu göstermiştir. Diyabetik hayvanlar ile yapılan çalışmalardan elde edilen bulgular da agomelatin'in 40 ve 80 mg.kg-1 dozlarda subakut (7 ve 14 gün) uygulamalarının nöropatiye bağlı olarak gelişen hiperaljezi ve allodini yanıtlarını anlamlı biçimde düzelttiğini ortaya koymuştur. Agomelatin tedavisi, hiperaljezi ve allodini yanıtlarını normoglisemik kontrol hayvanlarının seviyesine geri dönecek ölçüde düzeltmiştir. Antihiperaljezik ve antiallodinik etki açısından agomelatin referans ilaç pregabalin (10 mg.kg-1) kadar etkili bulunmuştur. Agomelatin'in antihiperaljezik ve antiallodinik etkileri de, akut antinosiseptif etkisine benzer biçimde, nalokson uygulaması ile ortadan kalkmıştır. Bu bulgu, bu ilacın tekrarlı uygulamalarının nöropatik ağrı üzerindeki terapötik etkinliğine de opioid mekanizmaların katıldığına işaret etmiştir. Bu tez çalışması ile yeni bir antidepresan olan agomelatin'in hem akut ağrı ve hem de diyabetik nöropatik ağrı üzerine opioid mekanizmaların aracılık ettiği güçlü terapötik etkinlik gösterdiği ilk kez ortaya konulmuştur. Agomelatin'in antidepresan, anksiyolitik ve antinosiseptif etkileri aynı anda taşıdığı göz önünde bulundurulduğunda, bu ilacın diyabetik nöropatik ağrının yanı sıra, diyabetik hastalarda insidansı oldukça yüksek olan depresyon ve anksiyete gibi duygu-durum bozukluklarını da aynı anda tedavi etmek gibi bir avantaja sahip olabileceği ortadadır. Bu durum klinisyenlere polifarmasiden kaçınma olanağı sağlaması açısından önemlidir.

Tez (yüksek lisans) - Anadolu Üniversitesi, Anadolu Üniversitesi, Sağlık Bilimleri Enstitüsü, Farmakoloji Anabilim Dalı, Kayıt no: 374556, Agomelatin, melatoninin MT? ve MT? reseptörlerine agonist ve serotoninin 5-HT?c reseptörlerine antagonist etki gösteren, kendine has etki mekanizmasına sahip yeni bir antidepresan ilaçtır. Bu tez çalışmasında agomelatin'in mekanik, termal ve kimyasal nosiseptif uyaran uygulamasına karşı antinosiseptif etkinliğinin taranması ve bu etkiye aracılık eden opioid reseptör alt-tiplerinin belirlenmesi planlanmıştır. Bu amaçla, sıcak plaka, kuyruk sıkıştırma ve asetik asid ile indüklenen kıvranma testleri yapılmıştır Agomelatin'in (40 mg.kg-¹ ve 60 mg.kg-¹), sıcak plaka ve kuyruk sıkıştırma testlerinde farelerin reaksiyon sürelerini anlamlı ölçüde artırmış olması, antinosiseptif aktivitenin supraspinal ve spinal mekanizmalar ile ilişkili olduğunu göstermiştir. Bu ilacın asetik asit kaynaklı kıvranma davranışlarının sayısını da anlamlı ölçüde azaltmış olması, antinosiseptif etkiye santral mekanizmaların yanı sıra periferik mekanizmaların da katıldığına işaret etmiştir. Farelerin Rota-rod testlerinde değerlendirilen motor koordinasyon değerlerinin değişmemiş olması, agomelatinin bu çalışmada ortaya konulan antinosiseptif etkisinin spesifik olduğuna işaret etmiştir. Nonselektif opioid reseptör antagonisti nalokson (5.48 mg.kg-¹, i.p)'un agomelatin ile indüklenen antinosiseptif etkiyi ortadan kaldırmış olması, söz konusu etkiye opioid mekanizmaların katıldığına işaret etmiştir. Antinosiseptif etkiye µ, ? ve ? reseptörlerinin olası katılımı sırasıyla naloksonazin (7 mg.kg-¹, s.c), naltrindol (0.99 mg.kg-¹, i.p) ve nor-binaltorfimin (1.03 mg.kg-¹, i.p) kullanılarak değerlendirilmiştir. Naloksonazin, naltrindol ve nor-binaltorfimin ön-uygulamaları, agomelatinin her üç nosiseptif testte de görülen antinosiseptif etkisini ortadan kaldırmıştır. Bu bulgular, antinosiseptif etkiye µ, ? ve ? reseptörlerinin her üçünün de katkıda bulunduğunu göstermiştir. Bu tez çalışması, agomelatinin µ, ? ve ? opioid reseptör aracılıklı akut antinosiseptif etkinliğinin gösterildiği ilk araştırma olma özelliğini taşımaktadır.

Prema DSM-V, dijagnostički kriteriji za sindrom noćnog jedenja (Night Eating Syndrome - NES) su: (1) ponavljano jedenje noću koje se manifestira nakon što se osoba probudi iz sna ili pretjerani unos hrane nakon večernjeg obroka ; (2) osoba je svjesna da jede i može se toga sjetiti ; (3) simptom se ne može bolje objasniti poremećajem ritma spavanja ili društvenim normama ; (4) posljedica jedenja noću je značajno uznemirenje i/ili oštećenje funkcioniranja ; (5) sindrom nije uzrokovan sindromom prejedanja (binge-eating disorder), drugim psihijatrijskim poremećejem i nije izazvan jatrogeno. Dosadašnja istraživanja farmakoterapijskog liječenja ovog sindroma potvrđuju samo terapiju selektivnim inhibitorima ponovne pohrane serotonina (SIPPS). Prikazujemo pacijenta u dobi od 25 godina koji se ambulantno liječi posljednje tri godine. Povod dolasku bile su somatske smetnje, poremećaj spavanja, porast tjeskobe i depresivnost. Svakodnevno funkcioniranje bilo je značajno narušeno. Pacijent je tijekom tri godine liječen različitim psihofarmacima. Postupno se postigne zadovoljavajuća remisija uz terapiju venlafaksinom, te unatrag pola godine pacijent nema klinički značajnih simpotma depresivnosti i tjeskobe. Unatrag godinu dana prisutno je učestalo buđenje iz sna i jedenje noću. Tijekom tog perioda pacijent je liječen različitim antidepresivima uključujući i SIPPS-e, no jedenje noću perzistira. Agomelatin je agonist melatonergičkih receptora (MT1, MT2) i antagonist serotoninskih (5-HT2c) receptora. Kako se sindrom noćnog jedenja povezuje s disbalansom melatonina, leptina i kortizola, odlučili smo se na uvođenje agomelatina u terapiju. Uz venlafaksin (XL - 150 mg ujutro) dodali smo agomelatin (Valdoxan) 25mg navečer. Nakon mjesec dana liječenja - jedenje noću je prestalo.

This is a protocol for a systematic review. The objectives are to determine the acute and long term efficacy of agomelatine in the treatment of unipolar major depression compared to placebo and to review the acceptability of agomelatine in comparison to placebo.

Urotensin II, is a vasoactive peptide found in all studied species. It presents a species-variable N-ending linear part and a constant hexocyclic C-ending part which is responsible for the activity. Among the developed ligands, Palosuran, a selective inhibitor of Urotensin II receptor, is interesting in the treatment of renal disorder in diabetic subjects or in some ischemic diseases. In the first part of this work, a succession of pyrido[2,3-d]pyrimidinic compounds has been developed, like those patended by Takeda and also based on the model of Palosuran.Melatonin is an indolic hormone produced during the night by the pineal gland and presenting many properties, the most important of which is the synchronization of the biological clock with the day and night cycle. Agomelatin, a naphthalenic melatoninergic analogue, developed by Laboratoires Servier for the treatment of depression, is an agonist of the MT1 and MT2 receptors and an antagonist of the 5HT2c receptor. On the basis of this model that pyrido[2,3-d]pyrimidine ligands have been developed by substituting the 2, 4 and 6 positions, either by alkylation or by palladium-assisted cross-coupling reaction. Given lateral-chain ramification of Agomelatin, which gave rise to active compounds, this led to the synthesis of pyrido[2,3-d]pyrimidine and imidazo[1,2-a]pyridine analogues bearing the same lateral chain.; L'Urotensine II est un peptide vasoactif que l'on retrouve chez toutes les espèces étudiées. Elle présente une partie N-terminale linéaire variable selon l'espèce et une partie C-terminale hexocyclique constante et responsable de l'activité. Parmi les ligands développés, le Palosuran, un inhibiteur sélectif du récepteur de l'Urotensine II, présente un intérêt dans le traitement de l'insuffisance rénale du patient diabétique voire dans certaines pathologies ischémiques. Dans la première partie de ce travail de thèse, une série de composés à noyau pyrido[2,3-d]pyrimidine a été développée, tout d'abord, par analogie aux structures brevetées par Takeda puis sur le modèle du Palosuran.La Mélatonine est une hormone à noyau indolique produite pendant la nuit par la glande pinéale et qui présente de nombreuses propriétés dont la plus importante est la synchronisation de l'horloge biologique avec le cycle jour-nuit. L'Agomélatine, analogue mélatoninergique à noyau naphtalénique développé par les Laboratoires Servier pour le traitement de la dépression, est un agoniste des récepteurs MT1 et MT2 et un antagoniste du récepteur 5HT2c. C'est sur ce modèle, qu'ont été développés des ligands à noyau pyrido[2,3-d]pyrimidine par substitution des sommets 2, 4 et 6, soit par alkylation, soit par couplages palladiés. La ramification de la chaîne latérale de l'Agomélatine ayant conduit à des composés actifs, il a été envisagé la synthèse d'analogues possédant la même chaîne en série pyrido[2,3-d]pyrimidine d'une part puis imidazo[1,2-a]pyridine d'autre part.
