Background: Several observational studies have reported the prevalence of cerebral microbleeds (CMBs) and their risk factors in an elderly population. Any information in this regard is currently lacking from India. Aim of this study was to estimate the prevalence, risk factors of CMBs, and association with cognition in an Indian urban population aged 50 years and above. Methods: Household surveys were conducted as part of ongoing Longitudinal Cognition and Aging Research on Population of the National Capital Region (LoCARPoN) study in areas of urban Delhi. Magnetic resonance imaging of the brain was performed in 2599 participants. Using standard neuropsychological battery, mean Z-scores for each domain (memory, executive, information) were derived. Binary and stepwise logistic regression models were used to determine associated risk factors for the presence of CMB and its association with cognitive domains. Results: The prevalence of CMBs was 14.42% (95% confidence interval [CI]: 13.06–15.73). Of these, 203 (7.81%) participants had single CMBs and 172 (6.61%) had multiple microbleeds (≥2). Higher prevalence was observed in older age (60–70 years: odds ratio [OR]: 1.25 [95% CI: 0.93–1.67]; 70–80 years: OR: 2.05 [95% CI: 1.48–2.84]; ≥80 years: OR: 3.27 [95% CI: 1.97–5.44]) compared to individuals in the age group 50–60 years. History of stroke (OR: 2.97 [95% CI: 1.56–5.66]), hypertension (OR: 1.36 [95% CI: 1.05–1.75]), and smoking (OR: 1.43 [95% CI: 1.11–1.85]) was associated with at least one CMB. Multiple CMBs were associated with worse scores in memory and executive domains. Conclusion: Older age, hypertension, history of stroke, and history of smoking emerged as important risk factors for the presence of multiple CMBs. Follow-up study is required to determine implications of CMBs.

Dermatomyositis (DM) and polymyositis are idiopathic inflammatory myopathies (IIMs), most associated with solid organ malignancies, and less commonly hematological malignancies. We discuss a case of DM associated with diffuse large B-cell lymphoma, followed by a review of literature on the pathogenesis, clinical course, treatment, and prognosis. Various challenges with the diagnosis and management of underlying lymphoproliferative disorders (LPDs) in patients with IIM are discussed. The case demonstrates the importance of being vigilant of the association between IIM and LPD. Cancer screening in patients with IIM is discussed, including the recently published International Guideline for IIM-Associated Cancer Screening. More research is required to address knowledge gaps in cancer screening in IIM.

Introduction: Acquired angioedema due to C1 esterase inhibitor deficiency (C1INH-AAE) is most associated with lymphoproliferative disorders (LPDs), particularly low-grade B-cell subtypes. The condition remains under-recognized with long diagnostic delays due to various challenges including a lack of awareness of the condition. Case Presentation: We discuss 4 cases of C1INH-AAE associated with low-grade B-cell LPDs, including various diagnostic and management challenges. As our cases illustrate, constitutional symptoms or overt manifestations of LPD at diagnosis are often absent. Hence, a comprehensive multimodal approach to screening for an underlying B-LPD is important when a diagnosis of acquired angioedema is made. Levels of complement C4, C1q, and C1INH are useful for diagnosing C1INH-AAE and for monitoring disease activity. Changes in these parameters may also indicate relapse of the underlying hematological malignancy. Treating the underlying disorder is important as this commonly leads to clinical improvement with decreased episodes of angioedema and normalization of complement studies. Conclusion: Awareness of C1INH-AAE can lead to an early diagnosis of hematological malignancies. The absence of constitutional symptoms emphasizes the need for a comprehensive multimodal approach to screening for LPD in C1INH-AAE. C4, C1INH level, and function are useful for monitoring disease activity.

Abstract Background Merkel cell carcinoma (MCC) is one of the deadliest skin cancers. Despite existing national guidelines, treatment of MCC patients is not as well standardized as for more common skin cancers. Objectives The study objective was to investigate factors predisposing to favourable/unfavourable patient outcomes and to which extent guideline‐based care affects patient survival. Methods This noninterventional study investigated a monocentre real‐world patient cohort with a histologically confirmed diagnosis of MCC who presented at the skin cancer centre, University Hospital Essen. Patient and tumour characteristics (age, sex, primary localization, Merkel cell polyomavirus [MCPyV], tumour stage at initial diagnosis and primary treatment measures [surgery, radiation]) were correlated with the patient outcome in terms of recurrence‐free survival (RFS) and overall survival (OS). Background: Altered patterns of amino acid profiles are observed in various pathological conditions including nutrition related disorders, cancer, diabetes, urea cycle defects, mitochondrial respiratory chain disorders, and aminoacidopathies. Aim and Objectives: To develop a cost effective Reverse Phase-High Performance Liquid Chromatography (RP-HPLC) method for the quantification of free amino acids in neonatal Dried Blood Spot (DBS) samples. Material and Methods: Free amino acids were extracted from neonatal DBS through elution with ice-cold methanol/2-mercaptoethanol. The extracted samples were then derivitized by using O-Pthalaldehyde (OPA)/2-mercaptoethanol, which resulted in the formation of OPA derivatives of amino acids. These derivatives were quantified by RP-HPLC. The developed method was validated according to the International Conference on Harmonization ICH Q2 (R1) Guidelines and applied to clinically confirmed aminoacidopathies. Results: A good linearity was observed from 2.5 to 1000 μM. The between run imprecision (on two different days) though varied over a wide range for different amino-acids, the Coefficience of Variation (CV) was found to be less than 5% (range; 0.1 to 5% for different amino acids) and the recovery was found to be around 82-125% (for spiked amino acids). The quantified concentration of amino acids in plasma and DBS sample were found to be almost similar except for few amino acids like glycine, tyrosine, alanine, methionine and lysine which showed higher values in dried blood spot samples. DBS amino acid concentrations determined by RP-HPLC showed concordance to the concentrations obtained through Tandem Mass Spectrometry (TMS) of DBS/plasma. Reference intervals of amino acids were established in term neonates. The proposed method was applied to the analysis of samples obtained from confirmed aminoacidopathies. Conclusion: The present approach with DBS offers a valid alternative to the plasma/serum aminoacid profiling in HPLC or DBS in Tandem mass method, with advantages like lower sample volume, feasibility, affordability, improved specificity and can be attempted in a simple diagnostic setup with acceptable accuracy and precision.

Introduction: The Government of India has formulated the Cigarettes and Other Tobacco Products Act (COTPA) to implement measures to ensure that effective protection is provided to nonsmokers from involuntary exposure to tobacco smoke. Bhubaneswar is the capital city of Odisha, India, was declared as “Tobacco Smoke Free City” in 2010. For strengthening the implementation of the COTPA Act, an effective regular assessment is needed, and hence, an observational study was planned to assess the current violations of Tobacco Smoking in Bhubaneswar. Materials and Methods: In this observational study, 416 different places were chosen from four different zones of Bhubaneswar. Data were collected with the help of Mobile enabled global positioning system (GPS) technology and pretested structured questionnaire. Results: In this study, 52.88% places were found to be having smoking violations. The study shows maximum violations have occurred in public places (90.89%) followed by transit places (75%). Violations were found to be very low in government buildings, educational, medical institutes, and hotel/restaurants. The average number of smokers in the city was found to be 4.90/place and the average number of smokers was found to be 4.37/public place. Conclusion: In this study, GPS-enabled Mobile App can be used to identify the different locations, where a violation of law occurs. This may help administrators to properly plan and implement the law. Even though Bhubaneswar was declared “Tobacco Smoke free city” in 2010, it is still lacking behind in fulfilling the implementation of law, to reduce Second Hand Smoking.

The present study was undertaken to investigate the effect of dietary replacement of commonly used vegetable oil (sunflower oil, SFO) with n-3 polyunsaturated fatty acids (PUFA) rich oil sources on broiler chicken performance, carcass yield, meat fatty acid composition, keeping quality and sensory attributes of meat. In the current experiment, 300 day-old Krishibro broiler chicks were randomly distributed to 5 dietary groups (50 replicates with 6 chicks in each) prepared by replacing SFO (2% and 3% of diet during starter and finisher periods, respectively) with n-3 PUFA rich soybean oil (SO), mustard oil (MO), linseed oil (LO) or fish oil (FO) on weight basis. Introduction: Ipsilateral floating hip and floating knee are very rare injuries. These injuries so uncommon that only three cases of similar kind have been reported. These injuries are due to high velocity injuries following motor vehicle accidents. Management of such complex injuries is a challenging task even in experienced hands as there are no standard treatment guidelines for such fractures. Case Report: We hereby report a 20 yr old male who sustained ipsilateral floating hip and ipsilateral floating knee injuries following motor vehicle accident. Patient was stabilized initially and later taken up for surgery. Patient was treated with interlocking nail for femur and tibia in the same sitting whereas acetabulam fracture was managed conservatively. At five months all the fractures united well with restoration of good range of motion in both hip and knee. Conclusion: Ipsilateral floating knee and floating hip are very rare injuries seen following high velocity motor vehicle accidents. There are no standard guidelines for treatment of those fractures as only a few cases of similar kind have been reported in literature. Background: Calcium sulfate belongs to synthetic ceramic group of bone graft substitutes having an osteoconductive property. Calcium sulfate is an easily available, economic bone graft substitute. It is a bio-inert material which over a period of weeks get resorbed and fibrovascular tissue takes its place which eventually allows neovascularisation and bone formation within the area. Use of calcium sulfate as a bone graft substitutes avoids the complications and morbidity associated with autograft like infection, second surgery, and finite amount of graft. Material and Methods: A total of 15 patients with benign bone lesions and chronic osteomyelitis were operated and the osseous defects were filled with calcium sulfate which was comprehensively sterilized .Patients were followed with serial radiographs at six week intervals. Calcium sulfate resorption and new bone formation was studied. Results: Thirteen cases out of 15 showed calcium sulphate resorption and new bone incorporation. Calcaium sulphate resorption occurred at an average of 14.5 weeks (range,13- 18weeks) whereas new bone incorporation occurred at an average of 6months ( range,5-7months). Thirteen patients out of these 15 cases returned to full activities of daily living. One patient had pathological fracture at the osseous defect postoperatively. One patient with GCT of distal end of tibia had recurrence of tumor and had to undergo second surgery. Conclusion: Although autogenous bone graft is the gold standard for bone grafting, it has limitations like finite amount, additional surgery and donor site morbidity. Calcium sulphate is safe, efficient and easily available bone graft substitute in the treatment of osseous defects. Most common complication encountered was aseptic serous discharge. Functional results were favorable in most of the cases.

Introduction: Bilateral shoulder dislocation are most commonly posterior type. These are most commonly due to seizure disorder and electrocution. Anterior shoulder dislocations occurring bilaterally without any predisposing factors are very rare. These types of injuries are due to trauma with a unique mechanism of injury. To best of our knowledge there are only few cases of similar kind are reported in literature. We hereby report a interesting case of posttraumatic, bilateral anterior dislocation of shoulder without associated fracture in a 45 old women without any predisposing pathoanatomy.Case Report: A 45-year-old women presented to casualty with sudden onset of pain and restriction of movement in both shoulders fallowing trauma. Immediately post trauma she had severe pain and restriction of both shoulders. On examination arms were abducted and externally rotated. Bilateral shoulder movements were painful and restricted . There was loss of round contour of shoulder with increased vertical diameter of axilla anteriorly. Radiological examination revealed bilateral anterior dislocation of the shoulders without any associated fractures. Closed reduction done by Milch technique after intraraticular lignocaine injection. MRI of bilateral shoulder showed no pathological lesion. Both shoulders were immobilized with a shoulder immobilizer for three weeks.Conclusion: Most of the bilateral shoulder dislocations are posterior type seen in seizure disorders. Bilateral traumatic anterior shoulder dislocations are rare and are seen as a result of unique mechanism of injury. In our case patient had a fall on her elbows causing forced extension. If diagnosed and treated promptly completely normal function of the shoulders can be restored.

Background: Degeneration can occur as a result of moisture loss and decreased flexibility in the neck's spinal discs over time. Cervical spondylosis can also be brought in addition to recurrent neck motions, prior neck injuries, bad posture, and genetic factors and by poor posture. Purpose: The study objective was comparison of the efficiency of interferential therapy and transcutaneous electrical nerve stimulation in construction workers having cervical spondylosis using the short form of the MC Gill pain questionnaire. Methodology: 50 subjects participated in the study from Mars India Builds and selected based on the inclusion and exclusion criteria. Subjects assigned into interferential therapy group (n=25) and transcutaneous electrical nerve stimulation group (n=25). Along with this, static neck exercises were given to both groups. The treatment period was given for 40 mins and 6 days per week and continued for 2 weeks. The entire study process is conducted from November 2022 to April 2023. Result: From the finding of this study interferential therapy group post-test mean was 8.88 and whereas the transcutaneous electrical nerve stimulation group was 11.72. This strongly suggests that interferential therapy in construction workers having cervical spondylosis along with static neck exercises is more effective than transcutaneous electrical nerve stimulation. Conclusion: In this study interferential therapy with static neck exercises among construction workers was found to be more effective than transcutaneous electrical nerve stimulation. The use of technology in education has grown crucial in today's classrooms all over the world. For a long time, technology has been a crucial tool for enhancing language learners' reading, writing, speaking, and listening skills. Thanks to technology, writing has become one of the most supported abilities. For learners, learning how to write paragraphs or essays is a crucial ability. The objectives of this experimental study were to: 1) assess the effectiveness of instructional design writing in enhancing ESL learners' writing abilities, 2) investigate students' satisfaction and motivation with instructional design writing, and 3) investigate students' autonomy following the completion of the instructional design writing course. 31 English literature undergraduate students participated in this study. The instruments used to acquire pertinent data were lesson plans, grammar and writing tasks, learner perception questionnaires, pre- and post-tests. The data was evaluated, and the results showed that the learners' writing achievement level was much greater after taking the course. The post-test scores of the students improved over the pre-test results. According to the findings, the students' motivation and level of satisfaction with the instructional design writing course were quite high. The results of this course also showed that the students' writing abilities might be enhanced. Suggestions are made and given in terms of the need for further study and practical use in the future. Instructional design, as a science, is relatively young with its roots being traced to military training during World War II. Instructional design saw a vibrant phase of development between the 1950’s and 70’s with a lot of brilliant fundamental research in the field, happening during the period. Starting with Programmed Instruction (Skinner, 1954), the most important theories and models that established ID as an applied science like Bloom’s Taxonomy (1956), Gagne’s Events of Instruction (1965) and ADDIE (1974) were founded during this period. With many new ID models being developed in the 1970s, the instructional design process grew very influential in military and industrial training. However, its application in the field of education gained significance only with the advent of computers in the 1980s. Theories of Cognitive Psychology and Constructivism were applied to ID in 1980s and 1990s. However, it was the integration of Information and Communications Technology (ICT) that gave ID, a firm foothold in the sphere of education. The internet in 2000 laid the foundation for the digital revolution in teaching and learning that we see today. The need for integration of ICT in Instructional Design saw development of various new ID models, modification of and alternative approaches to existing models. Besides content and pedagogy, technology became an important conduit of delivery in the new models like TPACK and its spinoffs. However, the research shows that the established models like ADDIE still enjoy the widest application with suitable integration of ICT. The need to customize training and education for the 21st century skills, a collective vision of the global business, educational and governing communities, is likely to see radical changes in the way instruction is designed and delivered in the days to come. The purpose of this study was to prepare and evaluate multiple unit mini tablets (MTs) of prolonged released (PR) bosentan (BSN) and fast release (FR) sildenafil citrate (SDC) on the basis of encapsulation method for the effective management of pulmonary artery hypertension. The system consists of MTs are in the form of uncoated tablets containing the superdisintegrating agents where as the PRMTs contains the polymer with film coating. All the units are prepared by direct compression method and encapsulated using size 1 hard gelatin capsule shells. The formulations were evaluated for differential scanning calorimetry, dissolution test and post compression studies for their quality attributes and it was found that all the parameters were in the acceptable limits. The optimized formulation for BSN PRMTs i.e BS3 which contains 5 % HPMC K15M shows a promising sustained release profile of 82.31 % in 24 h similarly FRMTs of SDC of formulation SD6 that having 3 % of magnesium aluminium silicate shows about 90 % of drug release within 15 min. The in-vivo pharmacokinetic characterization of the system was carried out using wister rats where the AUC value for SDC FRMTs and BSN PRMTs was found to be 94256.88625 ± 123.65 ng h/mL and 142438.084 ± 324.11 ng h/mL respectively. The other pharmacokinetics are also determined and found to be satisfactory. The six months stability samples showed no significant change in the drug content, hardness and uniformity of the content of the optimized formulations.

Surgical missions are often conducted in outreach areas where facilities are often not as advanced as in an institution. Excellent team coordination, detailed planning, and good communication are necessary for a successful outcome. Unfamiliar work environment, new acquaintances within the team, and language barriers at the mission site are some of the challenges often faced. Careful patient selection is crucial in a mission setup as most complications can be traced back to the preanaesthetic check and preoperative preparedness. We describe airway management strategies (airway assessment, mask ventilation, intubation options, and the postoperative airway plan) contrived by our anaesthesia team at one such surgical mission where four children with Pierre Robin Syndrome and incomplete cleft palate (involving posterior hard palate and complete soft palate)were successfully operated.

Abstract The growing popularity of nano-fertilization around the world for enhancing yield and nutrient use efficiency has been realized, however its influence on soil microbial structure is not fully understood. The purpose of carrying out this study was to assess the combined effect of nano and conventional fertilizers on the soil biological indicators and crop yield in a wheat–maize system. The results indicate that the at par grain yield of wheat and maize was obtained with application of 75% of recommended nitrogen (N) with full dose of phosphorus (P) and potassium (K) through conventional fertilizers along with nano-N (nano-urea) or nano-N plus nano-Zn sprays and N100PK i.e. business as usual (recommended dose of fertilizer). Important soil microbial property like microbial biomass carbon was found statistically similar with nano fertilizer-based management (N75PK + nano-N, and N75PK + nano-N + nano-Zn) and conventional management (N100PK), during both wheat and maize seasons. The experimental data indicated that the application of foliar spray of nano-fertilizers along with 75% N as basal is a sustainable nutrient management approach with respect to growth, yield and rhizosphere biological activity. Furthermore, two foliar sprays of nano-N or nano-N + nano-Zn curtailed N requirement by 25%, furthermore enhanced soil microbial diversity and the microbial community structure. The specific microbial groups, including Actinobacteria, Bacteroidia, and Proteobacteria, were present in abundance and were positively correlated with wheat and maize yield and soil microbial biomass carbon. Thus, one of the best nutrient management approaches for sustaining productivity and maintaining sound microbial diversity in wheat–maize rotation is the combined use of nano-fertilizers and conventional fertilizers.

The malignant transformation of chronic fistula in ano is rare, accounting for 3% to 11% of all anal canal malignancies. It results from long-standing inflammation and chronic irritation. No guidelines are available for the management of these cases. We herein present a case report of a 55-year-old man who presented with a history of constipation, perianal pain, and discharging fistula in ano of 4-year duration and underwent fistula surgery with recurrence. Biopsy of the fistulous tract revealed adenocarcinoma. He received neoadjuvant chemoradiotherapy, followed by abdominoperineal excision including excision of the fistulous tract. After 18 months of follow-up, he is free of recurrence. We present this case with a review of the literature, highlighting the management strategies.

Biometrics-based authentication systems offer obvious usability advantages over traditional password and token-based authentication schemes. However, biometrics raises several privacy concerns. A biometric is permanently associated with a user and cannot be changed. Hence, if a biometric identifier is compromised, it is lost forever and possibly for every application where the biometric is used. Moreover, if the same biometric is used in multiple applications, a user can potentially be tracked from one application to the next by cross-matching biometric databases. In this paper, we demonstrate several methods to generate multiple cancelable identifiers from fingerprint images to overcome these problems. In essence, a user can be given as many biometric identifiers as needed by issuing a new transformation "key." The identifiers can be cancelled and replaced when compromised. We empirically compare the performance of several algorithms such as Cartesian, polar, and surface folding transformations of the minutiae positions. It is demonstrated through multiple experiments that we can achieve revocability and prevent cross-matching of biometric databases. It is also shown that the transforms are noninvertible by demonstrating that it is computationally as hard to recover the original biometric identifier from a transformed version as by randomly guessing. Based on these empirical results and a theoretical analysis we conclude that feature-level cancelable biometric construction is practicable in large biometric deployments. Traditional fingerprint acquisition is limited to single-image capture and processing. With the advent of faster capture hardware, faster processors, and advances in video compression standards, newer systems can capture and exploit video signals for tasks that are difficult using a single image. In this paper, we propose the use of fingerprint video sequences to investigate detection of two aspects of dynamic behavior of fingerprints. Specifically, we are interested in the detection of distortion of fingerprint impressions due to excessive force and the detection of positioning of fingers during image capture. These two issues often lead to difficulties in establishing a precise match between images acquired from a finger. The proposed techniques use fingerprint video sequences to investigate dynamic characteristics of fingerprints across frames. Our approach directly works on MPEG-{1, 2} encoded fingerprint video streams. The inter-field flow estimate is used to investigate temporal characteristics of the behavior of the fingerprints. The joint temporal and motion analysis leads to reliable detection and characterization of relative finger position and pressure in streamed sequences. A significant advantage of our approach for distortion analysis is that it does not involve decompressing the video stream. The proposed methods have been tested on the NIST-24 live-scan fingerprint video database and the results are promising. We also describe a new concept called the 'resultant biometrics'--a new type of biometrics which has both a physiological, physical (e.g., force, torque, linear motion, rotation) component and/or temporal characteristic, added by a subject to an existing biometric. This resultant biometric is both desirable and efficient in terms of easy modification of compromised biometrics and is harder to produce with spoof body parts. One of the cancers that affect men, prostate cancer considerably raises mortality rates for males around the world. Patients with prostate cancer can have a localized or advanced form of the illness. Digital rectal examinations, prostate-specific antigen analyses, and prostate biopsies are all used to identify prostate cancer. The onset, development, and spread of cancer are all correlated with mutations in specific genes. Radical prostatectomy, ablative radiation, and active surveillance are all forms of treatment for localized prostate cancer. Androgen deprivation therapy (ADT), radiation, and chemotherapy are given to men who have metastatic prostate cancer or have experienced a relapse. When compared to traditional cancer chemotherapeutic methods, the liposome-based drug delivery technology offers less toxic, biodegradable, and biocompatible nanomedicine. Liposomes offer great advantages for use in nanomedicines by improving the sensitivity, specificity, and persistence of these anti-malignant cell agents in the body. Liposomal formulations are undergoing clinical trials of variety of cancers including prostate cancer. The present narrative review describes the composition and types of liposomes, its advantages, disadvantages, and the methods of preparation, research studies, clinical applications, drug repurposing and administration.

Objective: The laparoscopic approach is becoming the standard of care for many surgical disorders. However, in the case of hydatid cysts, laparoscopic management is challenging due to the risk of spillage of hydatid fluid, which can cause an anaphylactic reaction and recurrence. Here, we report our initial experience with laparoscopic partial pericystectomy of hydatid cysts using long ribbon gauze to decrease intra-operative spillage. Method: This was a retrospective study (between January 2010 and December 2021) in the Department of Surgical Gastroenterology, Sanjay Gandhi Postgraduate Institute of Medical Science, a tertiary care referral center in northern India. Here, we have included 37 consecutive patients with hydatid cysts of the liver and spleen. Diagnosis was made by laboratory and imaging findings (abdominal sonography or contrast enhanced CT scans). All patients were managed with laparoscopic partial pericystectomy. Intraoperatively, a betadine-soaked long ribbon gauze, high-pressure suction canula, and an endo-bag were used in all patients. The collected data included patient demography, location, size, and number of cysts, WHO type, operative time, blood loss, postoperative complications, hospital stay and follow-up. Result: In our series, the mean age was 38.4 ± 13.6 years, 15 (40.5%) were men and 22 (59.5%) were women. The right lobe of the liver was the most commonly affected site (21, 56.8%). The mean operative time was 80.0 ± 32.0 min, and intraoperative blood loss was 23.6 ± 11.5 mL. Bile leak was present in 6 (16.2%) patients. There was no mortality. The hospital stay was 5 (3, 9) days, and no recurrence was observed at a median follow-up of 36 months. Conclusion: Laparoscopic partial pericystectomy using this technique is safe in the management of hydatid cysts. Simply, proper packing and safe removal of soaked gauzes can minimize the incidence of postoperative complications and recurrence.
