Abstract

In the 50 years since the first mating of semiconductor-based energy-dispersive X-ray spectrometry (EDS) with the scanning electron microscope (SEM), this hybrid instrument has become an indispensable microanalytical tool. In the last two decades a new detector, the silicon drift detector (SDD), has overtaken earlier Si(Li) technology and has made EDS in the SEM and TEM faster and better. This article tells the story of the SDD development and describes improvements in count rate capability, energy resolution, and detector geometry that bring to SEM microanalysis exceptional precision and stability. Quality maps of element distributions can now be obtained in minutes instead of hours.

4. P123 TAPP - THE BEST APPROACH FOR INGUINAL HERNIA ASSOCIATED WITH ADULT CRYPTORCHIDISM: CASE REPORT AND LITERATURE REVIEW

Abstract

Aim Cryptorchidism is a rare condition of adult male. The diagnosis is based on examination: impalpable testis and CT/MRI. The surgical correction of cryptorchidism is possible in childhood, but in adult orchiectomy is recommended, considering the malignancy risk. The association with hernia is uncommon. Material and Methods We report the case of a 23 years old male with left inguinal hernia and concomitant left cryptorchidism. MRI revealed the intra-abdominal left testicle, with reduced size and short vessels. Lab analysis were normal. We underwent laparoscopic exploration: the left testicle was small and had pelvic localization. A large inguinal hernia was also confirmed. We decided to perform laparoscopic TAPP (transaabdominal preperitoneal repair) and left orchiectomy. The evolution was uneventful and patient discharged 1st day postoperatively. Histology revealed testicular atrophy with no malignancy, also no spermatogenesis was present. No recurrence was found during follow-up. Results Cryptorchidism in adult is rare. The association with hernia is even more rare. The open management is not easy, often in pelvic/abdominal localization, the inguinal approach can be cumbersome. There is no standard of care for these cases, but we found the minimally invasive approach to be a good choice. Laparoscopic surgery has the advantage of enhanced exploration and orhidectomy can be easily performed. Associated with the orchidectomy, hernia cure can be facilitated by laparoscopic TAPP procedure. Conclusions In cases of concomitant hernia and cryptorchidism, we emphasize the idea of simultaneously cure hernia via laparoscopic TAPP procedure associated with orchidectomy as feasible and safe.

Abstract

Background and Aims Registries provide an organized and standardized method to systematically collect observational data about specific groups of patients managed in routine clinical practice for a predetermined objective. Data collected by patient registries may help researchers develop hypotheses about disease mechanisms or treatment approaches and inform health care policy thereby potentially improving quality. It is known that chronic kidney disease (CKD) represents the 6th cause of mortality worldwide and it has an increased prevalence. Therefore, the aim of our Registry is to collect epidemiological and medical data regarding the CKD patients from our country in order to drive improvements in health care quality. Method Starting from January 2019, all Romanian nephrologist have been collecting demographic and medical data CKD-related from patients diagnosed with CKD stages 1-5. These data are introduced in a special designed software program – ReNal – that is easy to be accessed. The demographic and medical information include: ID, gender, age, region, body mass index, CKD etiology, comorbidities, chronic treatment, CKD stage, laboratory test, such as serum creatinine, urea, uric acid, calcium, phosphates, iPTH, albuminuria, proteinuria, glomerular filtration rate, hemoglobin etc. (results collected from each medical assessment). Results During almost 1-year, over 10000 patients with CKD stages 1-5 were introduced in our Registry, with mean age of 69.2 ± 3.5, 51.22% female patients, 48.78% male patients. From our preliminary results, hypertension and diabetes mellitus represent the main cause of CKD in Romania, according with the international data. In addition, CKD stages 3b and 4 were highly represented. These are part of the preliminary results, future data will be available. Conclusion Analyzing the assessed data from our Registry, in future several key points could be achieved: the importance of a correct financial appraisal sustained by health system, adequate and active assessment of CKD patients including the complications, highlighting the disease progression rate depending on the recommended treatment, designing an efficient prevention program.

Abstract

It is widely recognised in the social and management sciences that the effective support of decision-making requires a multidisciplinary perspective. This trend is also clear in nuclear emergency management (EM). However, communication between disciplines is not easy to maintain in EM contexts when the decision makers (DMs) are likely to be highly stressed. Such circumstances can lead them to revert to the instinctive patterns of perception of their core disciplines, making communication between disciplines difficult and, perhaps, obscuring complex interactions that have not been rehearsed in practice exercises. This paper explores decision making in EM and the nature of the socio-technical issues that will arise, suggesting that despite the lessons of past accidents the research EM community is still not taking a broad enough view of what future incidents may entail.
