1. Diffraction data analysis, microstructure and dielectric studies of transition metal doped LaCoO3

Abstract

LaCoO3 based compounds with 5% Cr and Fe ion as impurity at Co site were prepared by standard solid state reaction method. These sample were characterized for structural confirmation via XRD technique and for microstructural and compositional studies through SEM/EDAX characterization. In addition, these materials were investigated for dielectric properties. The X-ray diffraction studies infer that the sample are pure and single phased. These samples have acquired trigonal structure. The XRD result was verified via Rietveld refinement of pristine LaCoO3 compound. FTIR technique has been used to confirm the required oxide formation. The SEM images of these samples reveals that average grain size acquired after sintering is very high however the images present the porosity in the samples. EDAX data study confirmed that all the integral elements of the samples were retained and witnesses the absence of any foreign impurity. Dielectric study of these samples witness the samples exhibit high dielectric constant (capacitance) however the loss values are comparatively high subject to the modifications in microstructure.

Abstract

To evaluate patterns of use and outcomes of arteriovenous fistulas and prosthetic grafts within racial categories in a large population based cohort of hemodialysis (HD) patients in the United States.A retrospective analysis of white, black, and Hispanic patients in the prospectively maintained United States Renal Database System who had an autogenous fistula or prosthetic graft placed for HD access between January 2007 and December 2014 was performed. Analysis of variance, χThis study of 359,942 patients, composed of 285,781 autogenous fistulas (79.4%) and 74,161 prosthetic grafts (20.6%) placed in 213,877 white (59.4%), 115,727 black (32.2%), and 30,338 Hispanic (8.4%) patients. There was a 11% increase in the risk-adjusted odds of HD catheter use as bridge to autogenous fistula placement in blacks (adjusted odds ratio, 1.11; 95% confidence interval [CI], 1.08-1.14; P .001) and a 9% increase in Hispanics (adjusted odds ratio, 1.09; 95% CI, 1.05-1.14; P .001) compared with whites. Fistula maturation for HD access for whites vs blacks vs Hispanics was 77.0% vs 76.3% vs 77.8% (P = .35). After adjusting for covariates, fistula maturation was higher for blacks (adjusted hazard ratio, 1.09; 95% CI, 1.06-1.13; P .001) and Hispanics (adjusted hazard ratio, 1.13; 95% CI, 1.06-1.20; P .001) compared with whites. There was no significant difference in prosthetic graft maturation for blacks and Hispanics compared with whites. Primary, primary-assisted, and secondary patency were highest for Hispanic and least for black autogenous fistula recipients. Primary, primary-assisted, and secondary patency was also highest for Hispanic patients who received prosthetic grafts. Prosthetic grafts were associated with a decrease in patency and patient survival compared with fistulas in all racial categories. Mortality was lower for blacks and Hispanics relative to white patients. Initiation of HD with a catheter and conversion to autogenous fistula was associated with decrease in patency and patient survival compared with initiation with a fistula in all racial groups.Autogenous fistulas are associated with better patency and patient survival compared with prosthetic grafts for all races studied. The use of HD catheter before fistula placement is more prevalent in Hispanic and black patients and is associated with worse patency and patient survival irrespective of race. Fistula and graft patency is highest for Hispanic patients. Patient survival is higher for Hispanic and black patients relative to whites. These associations suggest potential benefit with initiation of HD via autogenous fistula and minimizing temporizing catheter use, irrespective of race.

Abstract

Multi-hole injectors for direct-injection spark-ignition engines offer atomisation benefits and flexibility in fuel targeting by selection of the number and orientation of the nozzle’s holes. However, the spray formation process from multi-hole injectors has been mostly studied for hot fuel conditions and no quantitative data really exist with fuel temperatures representative of engine operation at cold-start. The challenge is further complicated by the predicted fuel stock which will include a significant bio-derived component presenting the requirement to manage fuel sustainability. The thermophysical properties of bio-components like ethanol differ markedly from typical hydrocarbons found in gasoline like iso-octane. Moreover, the production of anhydrous ethanol fuel involves separating water and ethanol by way of distillation and dehydration in an energy intensive process. This work presents results from an optical investigation into the effect of atomisation for iso-octane, anhydrous ethanol, hydrous ethanol with 10% water content per volume and water. Tests were first carried out at ambient conditions of 20 °C, 1 bar with 150 bar injection pressure using high-speed spray imaging. Then the fuel temperature was lowered to −5 °C, −10 °C and −15 °C to focus on spray formation analysis at realistic cold-start engine conditions. Droplet sizing was also conducted by Phase Doppler Anemometry (PDA). The thermophysical properties of all fuels, including vapour pressure, density, viscosity and surface tension, as well as distillation curves, were obtained over a range of temperatures, and the Reynolds, Weber and Ohnesorge numbers were considered in the analysis. The results revealed the degree of suppression of atomisation for each type of fuel at cold temperatures and effects on spray penetration and cone angle, with ethanol fuels exhibiting the strongest effect in the form of longer initial delay out of the nozzle and shorter penetration.

Abstract

Abtbract Ethnopharmacological relevance Aconite root is very poisonous; causes cardiac arrhythmias, ventricular fibrillation and ventricular tachycardia. There is no specific antidote for aconite poisoning. In Ayurveda, dehydrated borax is mentioned for management of aconite poisoning. Aim of the study The investigation evaluated antidotal effect of processed borax against acute and sub-acute toxicity, cardiac toxicity and neuro-muscular toxicity caused by raw aconite. Materials and methods For acute protection Study, single dose of toxicant (35 mg/kg) and test drug (22.5 mg/kg and 112.5 mg/kg) was administered orally, and then 24 h survival of animals was observed. The schedule was continued for 30 days in sub-acute protection Study with daily doses of toxicant (6.25 mg/kg), test drug (22.5 mg/kg and 112.5 mg/kg) and vehicle. Hematological and biochemical tests of blood and serum, histopathology of vital organs were carried out. The cardiac activity Study was continued for 30 days with daily doses of toxicant (6.25 mg/kg), test drug (22.5 mg/kg), processed borax solution (22.5 mg/kg) and vehicle; ECG was taken after 1 h of drug administration on 1TB, 15th and on 30th day. For neuro-muscular activity Study, the leech dorsal muscle response to 2.5 µg of acetylcholine followed by response of toxicant at 25 µg and 50 µg doses and then response of test drug at 25 µg dose were recorded. Results Protection index indicates that treated borax gave protection to 50% rats exposed to the lethal dose of toxicant in acute protection Study. Most of the changes in hematological, biochemical parameters and histopathological Study induced by the toxicant in sub-acute protection Study were reversed significantly by the test drug treatment. The ventricular premature beat and ventricular tachyarrhythmia caused by the toxicant were reversed by the test drug indicate reversal of toxicant induced cardio-toxicity. The acetylcholine induced contractions in leech muscle were inhibited by toxicant and it was reversed by test drug treatment. Conclusion The processed borax solution is found as an effective protective agent to acute and sub-acute aconite poisoning, and aconite induced cardiac and neuro-muscular toxicity. Processed borax at therapeutic dose (22.5 mg/kg) has shown better antidotal activity profile than five times more than therapeutic dose (112.5 mg/kg).

Abstract

One of the approaches to address the issues of the cost of production, recovery and reusability of the extremozymes can be immobilization. In this report, we describe immobilization of an α-amylase from Laceyella sacchari TSI-2 and characterization of the immobilized enzyme. The enzyme was immobilized on 6 different matrices using entrapment, ionic binding and surface adsorption. The DEAE cellulose with glutaraldehyde crosslinking appeared most effective for the immobilization with high operational stability. While the temperature optima and thermal stability of the immobilized α-amylase shifted from 60 to 70°C with increased half-life, the pH optima remain unaltered while pH stability was shifted from 6 to 7. The stability of the immobilized enzyme improved in solvents. The enzyme catalysis in surfactants enhanced, while the Km and Vmax were reduced after immobilization. The structural features of the immobilized enzyme as probed by FT-IR established the role of aliphatic amines, esters and alkenes in immobilization. The starch hydrolysis efficiency of the immobilized enzyme was 15.55%. The immobilized enzyme in various detergents was highly efficient in removing the starch stain from cotton cloth. Taken together, the α-amylase turned more stable after immobilization and can be a favored choice for applications.

Abstract

A thermophilic actinobacterium, Laceyella sacchari TSI-2 from a hot spring was characterized for its morphological and cultural features. It optimally produced α-amylase at 50 °C and pH 7. The enzyme was purified by 55-fold with 12.23% yield and 4215.91 U/mg specific activity. The molecular weight, Km, Vmax and Kcat were 31 kD, 2.71 mg ml−1, 7.589 μmoles min−1 per mg of the protein and 4.31 × 10−2 s−1, respectively. Optimum catalysis and stability occurred at 70 °C and pH 7. A stabilizing effect of thiourea was evident, while metal ions and chelators inhibited the enzyme. The enzyme was highly stable in various surfactants. Kd, t1/2, ΔH*, ΔS* and ΔG* supported thermal stability reflected by the structural integrity and substrate affinity. The HPTLC analysis of the end products suggested the formation of malto-oligosaccharides as intermediate products along with maltose confirming the enzyme as α-amylase. The FT-IR revealed significant role of the triple bonds in thermal stability, while CD spectroscopy suggested changes in the secondary structure of the native and denatured enzymes. The enzyme was highly efficient in removing starch stains from the cotton cloth. Stability at high temperatures, alkaline pH and in surfactants suggests potential of the amylase in various applications.

Abstract

Despite the increasing prevalence of obesity and colonic diseases, the impact of obesity on short-term and oncologic outcomes of laparoscopic colorectal surgery still remains unclear.Seventy-six consecutive obese patients with body mass index (BMI) ≥30 kg/m(2) who underwent laparoscopic colectomy were matched with 76 nonobese patients with BMI30 kg/m(2). Perioperative parameters and oncologic outcomes were analyzed in the two groups.Obesity was associated with greater operative time (obese vs nonobese, 182 ± 59 vs 157 ± 55 min, P = .0084) and multivariate analysis identified BMI (hazard ratio 2.11, 95% confidence interval 0.64-3.56, P = .0049) as an independent predicting factor for operative time together with cancer location (hazard ratio 28.6, 95% confidence interval 14.62-42.51, P .0001). Obesity had no adverse influence on overall morbidity (25 vs 21%, P = .563), however, or postoperative duration of stay (median 6.0 vs 5.5 days, P = .22). Furthermore, the rate of conversion to open procedure was similar between the two groups (9 vs 9%, P .99). Regarding oncologic outcomes, there was no statistical difference in overall and disease-free survival between the two groups (5-year overall survival rate 86 vs 89%, P = .72, 5-year disease survival rate 70 vs 77%, P = .70).Laparoscopic colonic resection, when performed for selected patients, appears to be a safe and reasonable option in obese patients with colon cancer resulting in similar short-term and oncologic outcomes as nonobese patients.

14. Evolution of pancreatoduodenectomy in a tertiary cancer center in India: Improved results from service reconfiguration

Abstract

Pancreatic cancer incidence in India is low. Over the years, refinements in technique of pancreatoduodenectomy (PD) may have improved outcomes. No data is available from India, South-Central, or South West Asia to assess the impact of these refinements.To assess the impact of service reconfiguration and standardized protocols on outcomes of PD in a tertiary cancer center in India.Three specific time periods marking major shifts in practice and performance of PD were identified, viz. periods A (1992-2001; pancreaticogastrostomy predominantly performed), B (2003-July 2009; standardization of pancreaticojejunal anastomosis), and C (August 2009-December 2011; introduction of neoadjuvant chemo-radiotherapy and increased surgical volume).500 PDs were performed with a morbidity and mortality rate of 33% and 5.4%, respectively. Over the three periods, volume of cases/year significantly increased from 16 to 60 (p0.0001). Overall incidence of post-operative pancreatic anastomotic leak/fistula (POPF), hemorrhage, delayed gastric emptying (DGE), and bile leak was 11%, 6%, 3.4%, and 3.2%, respectively. The overall morbidity rates, as well as, the above individual complications significantly reduced from period A to B (p0.01) with no statistical difference between periods B and C.Evolution of practice and perioperative management of PD for pancreatic cancer at our center improved perioperative outcomes and helped sustain the improvements despite increasing surgical volume. By adopting standardized practices and gradually improving experience, countries with low incidence of pancreatic cancer and resource constraints can achieve outcomes comparable to high-incidence, developed nations.The manuscript represents the largest series on perioperative outcomes for pancreatoduodenectomy from South West and South-Central Asia - a region with a low incidence of pancreatic cancer and a disproportionate distribution of resources highlighting the impact of high volumes, standardization and service reconfiguration.

Abstract

Objective To review the evolution of traumatic thoracic aortic injury (TTAI) treatment at a single institution. Methods Retrospective analysis of all patients included in an institutional trauma registry and vascular surgery database who underwent treatment of TTAI between January 1999 and January 2011. Results Ninety-one patients (69 males) were treated for TTAI. The mean age was 38.5 years (range, 16-79 years). Forty-one patients underwent open repair (OR) and 50 thoracic endovascular repair (TEVAR), 37 with thoracic stent grafts (TSG) alone, 11 with infrarenal aortic extender cuffs (AEC), and two with a combination of TSG and AEC. OR was performed exclusively until 2004; the last one was performed in January 2007. All TTAIs have since been treated with TEVAR. The left subclavian artery (LSA) was fully covered in 10 patients (20%) and partially covered in eight patients, with revascularization in only two cases. The use of AEC and avoidance of LSA coverage increased after 2007. Baseline characteristics and injury severity scores were similar between groups. The mortality rate was higher in the OR group (19.5% vs 6.0%; P = .06), although it did not reach statistical significance. The overall incidence of morbidities was similar between the two groups (42% OR vs 50% TEVAR). Two patients developed paraplegia (4.4%) after OR compared with none after TEVAR. In the TEVAR group, a pseudoaneurysm, an iliac artery thrombosis, and a retroperitoneal hematoma developed in one patient each. Overall, eight patients (16%) developed stent graft-related complications (SRC), with two developing early (within 30 days) complications. All complications were related to poor apposition, requiring 10 reinterventions. Four patients underwent open conversions with no mortality. Nine out of 10 SRCs were associated with the use of thoracic stent graft malapposition. No patient treated with AEC had endoleaks or SRC. Conclusions TEVAR for TTAI has superior survival outcomes and has replaced OR. SRC requiring reintervention is associated with malapposition and the use of TSG. Until TTAI-specific endografts become available, use of AEC may minimize malapposition and reduce reinterventions. Routine overstenting of the LSA is not necessary and may increase SRC.

Abstract

Background The ideal treatment of patients with “borderline resectable pancreatic tumors (BRTs)” needs to be established. Current protocols advise neoadjuvant chemo (radio) therapy, although some patients may appear to have BRT on preoperative imaging and a complete resection may be achieved without the need for vascular resection. The aim of the present study was to identify specific findings on preoperative imaging that could help predict in which patients with BRT a complete resection, with or without vascular resection (VR), could be achieved. Methods Twelve patients with BRTs were identified. Tumor location, maximum degree of circumferential contact (CC), length of contact of the tumor with major vessels (LC), and luminal narrowing of vessels at the point of contact with the tumor (venous deformity, VD) were graded on preoperatively acquired multidetector computed tomography (MDCT) images and then compared with the intraoperative findings and need for VR. Results A complete resection (R0) was achieved in 10 patients with 2 having microscopic positive margins (R1) on histopathology at the uncinate margin. Four of the 10 patients required VR (40%). In 3 of the 4 patients whose tumors required VRs, CC was ≥ grade III and VD was grade 2. LC did not influence the need for VR. Conclusions It is possible to achieve a complete resection at the first instance in patients found to have BRTs on preoperative imaging. Preoperative MDCT-based grading systems and our proposed criteria may help identify such patients, thus avoiding any delay in curative resections in such patients.

Abstract

Identifying patients prior to treatment who are more likely to benefit from chemotherapeutic agents or more likely to experience adverse events is the aim of personalized medicine. Pharmacogenomics offers a potential means of achieving this through the discovery of predictive germline genetic biomarkers. When applied particularly to the treatment of head and neck cancers, such information could offer significant benefit to patients as a means of potentially reducing morbidity associated with platinum-based chemotherapy. We developed a genome-wide cell-based approach to identify single nucleotide polymorphisms (SNPs) associated with platinum susceptibility and then evaluated these SNPs as predictors for response and toxicity in head and neck cancer patients treated with platinum-based therapy as part of a phase II clinical trial. Sixty head and neck cancer patients were evaluated. Of 45 genome-wide SNPs examined, we found that two SNPs, rs6870861 (p=0.004; false discovery rate [FDR]

Abstract

Thoracic stent graft collapse is a rare complication of thoracic endovascular aortic repair that is mostly asymptomatic and occurs ≤ 3 months of the procedure. We describe the case of a 36-year-old man who presented with symptomatic endograft collapse 38 months after an initial thoracic endovascular aortic repair that was performed for traumatic aortic transection. He had sudden and complete loss of bilateral lower extremity motor and sensory functions (spinal cord ischemia) and anal sphincter tone. The patient was successfully treated with redo thoracic endovascular aortic repair, followed by open conversion and device explantation.

Abstract

Background The management of carcinoma in situ (CIS) of the penis is controversial, with relatively high local recurrence rates after minimally invasive therapies. Objective Report the surgical technique and outcome of partial glans resurfacing (PGR) and total glans resurfacing (TGR) as primary treatment modalities for CIS of the glans penis. Design, setting, and participants Between 2001 to 2010, 25 patients with biopsy-proven CIS underwent TGR ( n =10) or PGR ( n =15), defined as Surgical procedure Excision of the glans epithelium and subepithelium of either the entire glans or the locally affected area, with a macroscopic clear margin. The penis was then reconstructed using a split skin graft. Measurements Positive surgical margin (PSM) rates and rates of recurrence and progression were collated. Complications, cosmesis, and patient satisfaction were evaluated. Results and limitations Mean follow-up was 29 mo (range: 2–120 mo). There were no postoperative complications, and 24 of 25 patients (96%) had complete graft take with excellent cosmesis. Overall, 12 of 25 patients (48%) had PSMs. Only 7 of 25 (28%) required further surgery, 2 of 25 (8%) for extensive CIS at the margin and 5 of 25 (20%) for unexpected invasive disease. Additional surgery consisted of further resurfacing in 4 of 25 cases (16%) or glansectomy in 3 of 25 cases (12%). Those undergoing further surgery had no further compromise to their oncologic outcome. The overall local recurrence rate was 4%. There were no cases of progression. Conclusions Glans resurfacing is a safe and effective primary treatment for CIS. The procedure maintains a functional penis without compromising oncologic control, while ensuring that definitive histopathlogy is obtained. Glans resurfacing has a low risk of recurrence and progression. Patients need to be warned that approximately 28% will require further surgery for PSM or understaging of their primary disease, although the need for further surgery does not compromise oncologic control.

Abstract

The U.S. Tox21 collaborative program represents a paradigm shift in toxicity testing of chemical compounds from traditional in vivo tests to less expensive and higher throughput in vitro methods to prioritize compounds for further study, identify mechanisms of action, and ultimately develop predictive models for adverse health effects in humans. The NIH Chemical Genomics Center (NCGC) is an integral component of the Tox21 collaboration due to its quantitative high throughput screening (qHTS) paradigm, in which titration-based screening is used to profile hundreds of thousands of compounds per week. Here, we describe the Tox21 collaboration, qHTS-based compound testing, and the various Tox21 screening assays that have been validated and tested at the NCGC to date.

Abstract

Ferrite of system, namely Ni1−xZnxFe2O4 with x = (0.0, 0.2, 0.4, 0.6, 0.8, 1.0), have been prepared by solid state reaction to investigate the effect of gamma rays irradiations using Co60 source on the cation distribution, structural and magnetic properties. The unirradiated and irradiated samples were then subjected to characterization techniques such as X-ray diffraction, magnetization and AC susceptibility. The results of these characterizations are found to be different for irradiated from that of the pristine sample. The modifications in respect of irradiated samples are explained in terms of the ion-induced disorder. The important result of γ-irradiation on the cation distribution, structural and magnetic properties is the change of ratio Fe2+/Fe3+. Possible reasons on the results are proposed.

Abstract

Nuclear factor-kappa B (NF-kappaB) is a transcription factor that plays a critical role across many cellular processes including embryonic and neuronal development, cell proliferation, apoptosis, and immune responses to infection and inflammation. Dysregulation of NF-kappaB signaling is associated with inflammatory diseases and certain cancers. Constitutive activation of NF-kappaB signaling has been found in some types of tumors including breast, colon, prostate, skin and lymphoid, hence therapeutic blockade of NF-kappaB signaling in cancer cells provides an attractive strategy for the development of anticancer drugs. To identify small molecule inhibitors of NF-kappaB signaling, we screened approximately 2800 clinically approved drugs and bioactive compounds from the NIH Chemical Genomics Center Pharmaceutical Collection (NPC) in a NF-kappaB mediated beta-lactamase reporter gene assay. Each compound was tested at fifteen different concentrations in a quantitative high throughput screening format. We identified nineteen drugs that inhibited NF-kappaB signaling, with potencies as low as 20 nM. Many of these drugs, including emetine, fluorosalan, sunitinib malate, bithionol, narasin, tribromsalan, and lestaurtinib, inhibited NF-kappaB signaling via inhibition of IkappaBalpha phosphorylation. Others, such as ectinascidin 743, chromomycin A3 and bortezomib utilized other mechanisms. Furthermore, many of these drugs induced caspase 3/7 activity and had an inhibitory effect on cervical cancer cell growth. Our results indicate that many currently approved pharmaceuticals have previously unappreciated effects on NF-kappaB signaling, which may contribute to anticancer therapeutic effects. Comprehensive profiling of approved drugs provides insight into their molecular mechanisms, thus providing a basis for drug repurposing.

Abstract

Background There have been an increasing number of reports world-wide relating improved outcomes after pancreatic resections to high volumes thereby supporting the idea of centralization of pancreatic resectional surgery. To date there has been no collective attempt from India at addressing this issue. This cohort study analysed peri-operative outcomes after pancreatoduodenectomy (PD) at seven major Indian centres. Materials and Methods Between January 2005 and December 2007, retrospective data on PDs, including intra-operative and post-operative factors, were obtained from seven major centres for pancreatic surgery in India. Results Between January 2005 and December 2007, a total of 718 PDs were performed in India at the seven centres. The median number of PDs performed per year was 34 (range 9–54). The median number of PDs per surgeon per year was 16 (range 7–38). Ninety-four per cent of surgeries were performed for suspected malignancy in the pancreatic head and periampullary region. The median mortality rate per centre was four (range 2–5%). Wound infections were the commonest complication with a median incidence per centre of 18% (range 9.3–32.2%), and the median post-operative duration of hospital stay was 16 days (range 4–100 days). Conclusions This is the first multi-centric report of peri-operative outcomes of PD from India. The results from these specialist centers are very acceptable, and appear to support the thrust towards centralization.

Abstract

Polycrystalline ferrites with general formula Co 1− x Zn x Fe 2 O 4 ( x = 0, 0.2, 0.4, 0.6, 0.8, and 1.0) were prepared by solid-state reaction technique. The structure and morphology of obtained products were investigated by X-ray diffraction (XRD) and scanning electron microscope (SEM) techniques. The analysis of XRD pattern revealed the formation of single-phase cubic spinel structure. Lattice constant increases with increase in zinc content, obeying Vegard's law. Elastic properties such as longitudinal velocity ( V l ), transverse velocities ( V t ), Poisson's ratio ( σ ) and Debye temperature ( θ D ) have been studied. In the present work the elastic properties decreases with zinc composition x whereas Poisson's ratios almost remain constant. Debye temperature calculated from elastic data decreases with Zn substitution. A probable explanation has been proposed to explain the effect of zinc substitution on structural and elastic properties of cobalt ferrite.

Phytochemical study of its aerial parts led to isolation of seven pregnane glycosides (hoodigosides W–Z, hoodistanalosides A–B). Their structures were elucidated by chemical degradation studies and spectroscopic methods, including 1D and 2D NMR and CD spectroscopic methods.

Abstract

Background and aim. There is a need to increase the available data on revision radical surgery for incidental gallbladder cancer and to determine factors influencing operability. We aimed to assess the impact of stage of disease (pT) and the type of primary surgery (laparoscopy versus open) on resectability rates. Material and methods. The data of 90 consecutive patients referred to the Tata Memorial Hospital between 1 January 2003 and 30 April 2007 for revision radical surgery for incidental gallbladder cancer were reviewed retrospectively. Results. Of the 90 patients who underwent revision surgery, accurate data on T-stage was available in 76, and of these 76 patients, 44 (57.8%) had prior laparoscopic simple cholecystectomy, while 32 (42.2%) had undergone open surgery. The median time interval between the two surgeries was 2 months (range 4 weeks to 11 months). By T-stage, 23 patients had T1b disease, while 33 and 20 patients had T2 and T3 disease, respectively. Successful revision surgery could be undertaken in 71% of patients (54/76) and 29.6% of these had residual disease confirmed by histopathological examination. Conclusions. T-stage is an important factor in determining operability as confirmed by our study. As the T-stage of the disease increased, the chances of finding residual disease increased, while operability decreased. Furthermore, the case for revision surgery is strengthened because the incidence of lymph nodal disease is high even for pT1b cancers. The type of primary surgery does not affect operability in patients undergoing revision radical surgery for incidental gallbladder cancer.

The pathogenesis of gallbladder cancer presenting synchronously with malignancy of the bile duct has not been clearly understood. The possible causes for the simultaneous presence of these tumors could be due to local spread, metastases, de novo multifocal origin, or as part of a field change in the extrahepatic biliary apparatus. In this article, we discuss the cases of four patients with simultaneous gallbladder and bile duct malignancies and analyze their individual pathologies to provide an explanation into the mechanisms that may play a role in such conditions.

Pancreatic carcinoma accounts for the most dismal survival among all malignancies with 5-year survival rates approaching 5%. The reason for this, besides the inherent biologic nature of the disease, is the fact that the patients tend to present late in the disease. We present a review of the current published data on cystic neoplasms of the pancreas, which though rare, constitute an important subgroup of pancreatic neoplasms that have a better prognosis and are potentially curable lesions.

Abstract

Monoclonal antibodies (MAbs) were generated against the recombinant plasminogen activator (Pla) protein of Yersinia pestis. These MAbs detected Pla in all the 18 isolates of Y. pestis obtained from the sputum of pneumonic plague patients and from the liver and spleen of rodents from plague-affected areas of India during 1994–1995 as well as in seven of the eight isolates obtained from rodents in the surveillance regions of Hosur and Palmner in India during 1998 by simple dot-ELISA. In immunoblotting, the MAbs reacted with the Pla antigen only in Y. pestis isolates at 37 and 35 kDa region. These monoclonal antibodies, being strictly specific, can be used for detecting Y. pestis isolates that are Fraction 1 antigen-negative. Also, the radiolabelled pla fragment hybridized specifically to the representative DNA samples of Y. pestis isolates.

Antibiotics are an important class of therapeutic agents, which are used for the treatment of bacterial infectious diseases in a variety of animal species. Antibiotic therapy varies from treatment period to administration routes, depending on the animal species or the type of the disease being treated. Despite the fact that there are a wide variety of commercially available antibiotics, difficulties and problems associated with the administration of antibiotics to animals still exist. Thus, there is a great need and tremendous opportunity to develop long-acting antibiotic formulations for veterinary applications. In this review article, common approaches used to develop long-acting antibiotic formulations are summarized. The challenges and issues related to the development of these long-acting formulations are also discussed.

The purpose of this study was to examine the potential use of electrolytes to control naproxen sodium (I) release from chitinosan (II) tablets. An ANOVA was employed to evaluate the effects of molecular weight (MW) of II, electrolyte valence (EV), and pH of the dissolution medium on I's release. The intrinsic dissolution rates and saturation solubilities of I were determined at each of the pHs used. Directly compressed tablets were prepared from admixtures containing: I, NaCl, CaCl(2), or AlCl(3), Mg stearate, and II. The tablets were characterized for their dimensions, crushing strengths, friability, disintegration times, and in vitro dissolution profiles. The slopes of the log-log cumulative percent released-time curves (t=0-5 h) were compared using ANOVA. Based on the ANOVA, each of the variables-chitinosans, EVs, and pHs-significantly affected drug release (P0.05). Besides the poor aqueous solubility of I, the factors possibly affecting drug release included: (a) the formation of a rate-limiting II gel barrier; (b) the interaction of I with ionized amino groups of II; (c) the effect of electrolyte on the II's gel barrier formation; and/or (d) decreased aqueous solubility of I in the presence of electrolyte.

Abstract

Hepatopancreatobiliary (HPB) surgery is technically challenging and involves a lengthy learning curve.1,2 The complexity of the procedures involved not only requires the surgeon to have the technical skill needed to perform the surgery, as well as expertise in perioperative management, but demands a multi-disciplinary approach. The practice of carrying out major HPB surgery in high-volume centres has certainly helped improve outcomes.3,4 However, despite the apparent reduction in mortality rates following HPB surgery, morbidity remains high, indicating that improvement is needed not only in the procedures themselves, but also in the process of practice of HPB surgery. Broadly, the standardization of operative technique5 and uniformity of practice in the form of protocol generation based on sound evidence can aid such a thrust towards procedural and process improvement. However, the path to success is never easy and factors such as the difficulties involved in training surgeons in HPB surgery2 must be considered. In short, although recent times have witnessed considerable improvements in outcomes in HPB surgery, we need to strive for overall excellence. The Six Sigma methodology is a well-known business management tool that might contribute towards the realization of this goal.

Abstract

The near infrared (NIR) spectroscopic technique was used to determine copolymer ratios of polylactide-co-glycolide samples. Appropriate quantities of dl -polylactic acid and lactic-co-glycolic acid polymers with 86:14, 75:25, 64:36 and 52:48 lactide to glycolide ratios were dissolved in methylene chloride to obtain 5% (w/w) solutions. NIR spectra of the samples were obtained from the solutions using a Polyol Analyzer™ operated in the transmittance mode. Linear regression calibration models were generated at 2130 and 2288 nm from the second derivative spectral data obtained from the NIR technique. The lowest and highest standard errors of calibration (SEC) at 2130 nm were 1.29 and 1.63%, whereas those obtained from the calibration models generated at 2288 nm were 2.00 and 2.03%, respectively. Partial least squares (PLS) calibration models were also generated from the second derivative spectral data from 1100 to 2500 nm. The lowest and the highest SEC for the models were 1.46 and 1.53%, respectively. The calibration models were then used to predict the lactide contents of the unknown (test) samples. The highest percent error of prediction was 2.56% for samples with 86% lactide content when the linear regression calibration at 2130 nm was used, whereas the highest percent error of prediction was 1.56% for samples with 64% lactide content when the linear regression calibration at 2288 nm was used. The highest percent error of prediction was 1.73% for samples with 75% lactide content when the two-factor PLS calibration model was used.

Abstract

The structural and magnetic properties of Mg x Co 1− x FeCrO 4 ( x =0.0, 0.1, 0.2, 0.3, 0.4, 0.5 and 0.6) system have been investigated by X-ray diffraction, magnetization, AC susceptibility and Mossbauer spectroscopy techniques. Magnetization measurements exhibit Neel's collinear ferrimagnetic structure for the range studied, which is further supported by variation of Neel temperature with Mg-content. The Neel temperatures calculated theoretically are in agreement with those deduced experimentally from AC susceptibility measurements. Mossbauer results suggest existence of two types of spin–spin relaxation, smaller magnetic clusters relaxing at a faster rate as compared to the larger ones and giving rise to a quadrupole doublet for x

Abstract

We report the result of a search for scalar leptons in e+e−collisions at 183 GeV centre-of-mass energy at LEP. No evidence for such particles is found in a data sample of 55.2 pb−1. Improved upper limits are set on the production cross-sections for stable as well as unstable scalar leptons. New exclusion contours in the parameter space of the Minimal Supersymmetric Standard Model are derived, as well as new lower limits on scalar lepton masses.

Abstract

The term 'chitinosans' embraces the spectrum of acetylated poly(N-glucosamines) ranging from chitin to chitosan. Chitinosans (I), at acidic pH, have protonated amines which can interact with oppositely charged drug ions and, thereby, modify drug release from drug delivery systems. Tablets were compressed from a physical mixture containing salicylic acid (II) as the model drug, I, and magnesium stearate. Five commercial I compounds, varying in degree of deacetylation and molecular weight, were selected. Tablets were compressed at 5000, 10 000, and 15 000 psig using a Carver and a single punch tablet press. The differential scanning calorimetry thermograms provided evidence of I-II interaction in the powder blend. Analysis of variance (ANOVA) indicated that the compression pressure did not significantly affect the crushing strength (CS) or the release profile of II from the I-matrix tablets (P?0.05). Furthermore, the ANOVA also indicated that the tablet press used during manufacture did not affect the above properties (P?0.05); however, the chitinosans significantly affected the CS as well as the release profile of II from I-matrix tablets (P

Abstract

The time-integrated $\mathrm{B}^{0}\bar{\mathrm{B}}^{0}$ mixing parameter and the forward-backward charge asymmetry in the process $\mathrm{e}^{+}\mathrm{e}^{-} \rightarrow \mathrm{b}\bar{\mathrm{b}}$ are measured in hadronic \Z events containing prompt muons or electrons, collected by the L3 experiment in the years 1990 to 1995. The total sample of 3.3 million hadronic $\mathrm{Z}$ events with a mean centre-of-mass energy of 91.26 GeV yields: \begin{eqnarray*} \mbox{$A_{\mathrm{FB}}^{\mathrm{b} }$} & = & 0.0960 \pm 0.0066(stat.) \pm 0.0033(sys.) \,, \\ %corel-6 \mbox{$\chi_{\mathrm{b}}$} & = & 0.1192 \pm 0.0068(stat.) \pm 0.0051(sys.) \,. % \end{eqnarray*} %%% %%% %%% %%% This asymmetry measurement together with measurements at energies away from the $\mathrm{Z}$ pole energy yield a pole asymmetry and corresponding effective electroweak mixing angle of: \begin{eqnarray*} \mbox{$A_{\mathrm{FB}}^{0,\mathrm{b}}$} & = & 0.1015 \pm 0.0064(stat.) \pm 0.0035(sys.) \,, \\ %corel-6.nb \mbox{$\sin^{2}\bar{\theta}_{\mathrm{W}}$} & = & 0.2318 \pm 0.0013 \, .\\ %sin2tw-3.nb \end{eqnarray*}

Abstract

We report on a study of single W boson production in a data sample collected by the L3 detector at centre-of-mass energies from 130 to 183 GeV. The signal consists of large missing energy final states with a single energetic lepton or two hadronic jets. The measured cross sections at five different centre-of-mass energies are consistent with the Standard Model expectations. The following limits on the anomalous WW γ gauge couplings are derived at 95% CL: −0.46 Δκ γ λ γ

Abstract

We report on the first test of CP invariance in Z decays with hard photon radiation. The data recorded with the L3 detector at centre-of-mass energies near the Z resonance are used to search for CP violation in the reaction e+e-→ μ+μ-γ. Abstract

Solid pseudopapillary neoplasm of pancreas (SPN) is a rare entity; therefore proper diagnosis, evaluation and formulation of treatment protocols is difficult. A total of 14 cases were diagnosed and treated at our institute over a 10‐year period (1992–2002). A retrospective study of all these cases was performed with respect to age group most frequently affected, sex ratio, common presenting symptoms and signs, investigative protocols, pathological features, treatment offered, outcome and prognosis. All the 14 patients were females, with a median age of 20 years (range 13–45 years). The commonest presenting symptom was abdominal pain. All these patients underwent surgical exploration; 13 patients underwent resection of the tumour. The average length of postoperative hospital stay was 10.3 days. Significant postoperative morbidity was seen in only one case. There was no postoperative mortality. All these patients who underwent resection were disease‐free on follow‐up ranging from 6 to 96 months (median 36 months).

Abstract

A search for single and multi-photon events with missing energy is performed using data collected at centre-of-mass energies between 161 GeV and 172 GeV for a total of 20.9 pb$^{-1}$ of integrated luminosity. The results obtained are used to derive the value for the $\nu\bar{\nu}\gamma(\gamma)$ cross section as well as upper limits on new physics processes.

Abstract

We have studied the process $e^+e^-{\rightarrow}\rm n {\gamma}$ $(\rm n{\ge}2)$ at centre-of-mass energies of 161.3 GeV and 172.1 GeV. The analysis is based on a sample of events collected by the L3 detector in 1996 corresponding to total integrated luminosities of 10.7 ${\rm pb^{-1}}$ and 10.1 ${\rm pb^{-1}}$ respectively. The observed rates of events with two and more photons and the characteristic distributions are in good agreement with the Standard Model expectations. This is used to set lower limits on contact interaction energy scale parameters, on the QED cut-off parameters and on the mass of excited electrons.

Abstract

A search for unstable neutral and charged heavy leptons as well as for stable charged heavy leptons has been made at center-of-mass energies s = 161 GeV and s 172 GeV with the L3 detector at LEP. No evidence for their existence was found. We exclude unstable neutral leptons of Dirac (Majorana) type for masses below 78.0 (66.7), 78.0 (66.7) and 72.2 (58.2) GeV, if the heavy neutrino couples to the electron, muon or tau family, respectively. We exclude unstable charged heavy leptons for masses below 81.0 GeV for a wide mass range of the associated neutral heavy lepton. The production of stable charged heavy leptons with a mass less than 84.2 GeV is also excluded. If the unstable charged heavy lepton decays via mixing into a massless neutrino, we exclude masses below 78.7 GeV.

Abstract

We report on measurements of $\mathrm{e^+e^-}$ annihilation into hadrons and lepton pairs. The data have been taken with the L3 detector at LEP at centre--of--mass energies between 161~$\mathrm{Ge\kern -0.12em V}$ and 172~$\mathrm{Ge\kern -0.12em V}$. In a data sample corresponding to 21.2~pb$^{-1}$ of integrated luminosity 2728 hadronic and 868 lepton--pair events are selected. The measured cross sections and leptonic forward--backward asymmetries agree well with the Standard Model predictions. \end{abstract}

Abstract

We present a study of the structure of hadronic events recorded by the L3 detector at LEP at the center of mass energies of 161 and 172 GeV. The data sample corresponds to an integrated luminosity of 21.25 pb-1 collected during the high energy runs of 1996. The distributions of event shape variables and the energy dependence of their mean values are well reproduced by QCD models. From a comparison of the data with resummed second order QCD calculations, we determine the strong coupling constant at the two energies. Combining with our earlier measurements we find that the strong coupling constant decreases with increasing energy as expected in QCD.

Abstract

A search for excited leptons $\rm e^*,~\mu^*, ~\tau^*$ and $\nu_{\rm e}^*$ in $\rm e^+e^-$ collisions at $\rm \sqrt{s}$ = 161 GeV is performed using the $\rm 10.8 ~pb^{-1}$ of data collected by the L3 detector at LEP. No evidence has been found for their existence. From an analysis of the expected $\ell^*\ell^*$ pair production in the channels $\rm ee\gamma\gamma$, $\rm \mu\mu\gamma\gamma$, $\rm \tau\tau\gamma\gamma$, $\rm eeWW$, and $\rm \nu\nu\gamma\gamma$, the lower mass limits at 95\% C.L. are 79.7 GeV for $\rm e^{*}$, 79.9 GeV for $\mu^{*}$, 79.3 GeV for $\tau^{*}$ and 71.3 GeV for $\nu_{\rm e}^{*}$ assuming the same couplings as for standard leptons. From an analysis of the expected $\ell\ell^*$ single production in channels $\rm ee\gamma$, $\mu\mu\gamma$, $ \tau\tau\gamma$, $\rm \nu_{e}eW$ and $\nu\nu\gamma$, the upper limits on the couplings $\lambda/m_{\ell^*}$ up to $m_{\ell^{*}}$ = 161 GeV are determined.
