The anastomosis between the posterior superior alveolar artery (PSAA) and the infraorbital artery (IOA) courses along half of the lateral wall of the maxillary sinus. Risk of injury to the anastomosis between PSAA and IOA during surgical procedures has been reported. The aim of the present study was to carry out a systematic review and meta-analysis to determine the frequency, location, and diameter of the anastomosis between PSAA and IOA in imaging studies (cone-beam computed tomography, spiral cone-beam computed tomography, and computed tomography). A search was carried out in the PubMed, EMBASE, and LILACS. Original works were included reporting imaging studies to analyze the frequency, location, and diameter of the anastomosis in humans. The risks of bias were analyzed using the AQUA tool. Generalized linear mixed models were used to estimate the frequency and the 95% CI in the meta-analysis. The variance in prevalence estimates was stabilized by logit transformation. The qualitative analysis included 49 studies with 10,837 patients. The frequency of the anastomosis between PSAA and IOA was 74% (prediction interval 0.20-0.97%). The anastomosis was most frequently located in the intraosseous region (60.0%), followed by the submembranous region (33.0%), and least frequently in an extraosseous location (5.0%). Arteries with diameter of 1.0-1.9 mm were most frequent (32.0%), followed by arteries with diameter less than 1 mm (23.0%); a small number with diameter greater than 2 mm was recorded (4.0%). Objectives: The posterior superior alveolar artery (PSAA) and the infraorbital artery (IOA), both of which are ultimate branches of the maxillary artery, are connected by a horizontal anastomosis. PSAA anastomoses intraosseously and extraosseously with IOA. Profuse bleeding from the lateral wall while performing the direct sinus augmentation is a significant intraoperative complication.Thus the present study focused to assess the location of an alveolar antral anastomosis (AAA) in relation to the crest of the alveolar bone using cone beam computed tomography (CBCT)., Methods: A total of 200 CBCT scans of patients who were indicated for implant surgery were chosen and assessed. Group 1 includes 100 dentate patients and Group 2 includes 100 partially edentulous patients. The location of anastomosis along the lateral wall of the maxillary sinus was evaluated in association with alveolar bone height with respect to three posterior maxillary teeth: first premolar, second premolar, and first molar., Results: The mean distance for P1, P2, and M1 was 21.94 ± 1.02 mm, 19.41 ± 0.40 mm, and 17.36 ± 0.51 mm, respectively, in the dentate group, whereas in the edentulous group, it was 20.07 ± 0.46 mm, 18.95 ± 0.32 mm, and 16.08 ± 0.16 mm. In 80% of participants, the distance of an AAA from the alveolar crest was between 16 and 23 mm, whereas in 12% of the participants the distance of an AAA from the alveolar crest was less than 16 mm., Conclusion: The present study concludes that the first premolar region is safe for preparing the lateral window but for the second premolar and first molar additional care should be taken prior to surgery., (© 2022. This study aimed at assessing the prevalence of alveolar antral artery (AAA) detection by CBCT, its related variables, and at describing explanatory models useful in surgical planning, by retrospective evaluation of CBCT explorations. The modelling of the probability for detecting AAA was undertaken using logistic generalized additive models (GAM). The capacity for discriminating detection/no detection was assessed by receiver operating characteristic curves. A total of 466 sinuses were studied. Univariate models showed detection probability was linked to sinus width and thickness of the lateral bony wall, together with the shape and height of the osseous crest. AAA detection probability increased steadily until the thickness of the bony wall reached 6 mm. Multivariate models resulted good discriminators for AAA detection, particularly for females, showing an area under the curve (AUC) of 0.85. Models considering patients altogether, and those including only males offered slightly lower values (AUC = 0.79). The probability of AAA detection by CBCT was influenced by gender (higher in males and for narrow sinuses) and increases with the thickness of the sinus lateral bony wall and the height of the residual alveolar ridge. Besides, and particularly for women, the thickness of the ridge at the basal level seems to improve the explanatory model for AAA detection.

Purpose: Understanding anatomical variations of the facial artery and its branches is important for dental and medical practitioners., Methods: Routine cadaveric dissection of the head and neck was performed to demonstrate the origin and branches of the facial artery., Results: Facial artery emerged from a common linguofacial trunk off the external carotid artery. On the face, the facial artery first gave off a pre-masseteric branch. Immediately after, an aberrant artery emerged from the facial artery that coursed along the ramus of the mandible, which upon further dissection and examination was found to anastomose with inferior alveolar artery within the ramus of the mandible., Conclusions: We report a unique anastomosis between facial and inferior alveolar arteries, vessels that have not been previously shown to communicate. This case report may provide useful information for oral and maxillofacial surgeons as well as dentists performing inferior alveolar nerve blocks.

In our previous study, we revealed significant differences of osteopontin (OPN) gene expression in primary human osteoblasts (HOBs) derived from iliac crest bone (iHOBs) and alveolar bone (aHOBs). The present study aims at assigning this discriminative expression to a possible biologic function. OPN is known to be involved in several pathologic and physiologic processes, among others angiogenesis. Therefore, we studied the reaction of human umbilical vein endothelial cells (HUVECs) to HOB-derived OPN regarding angiogenesis. To this end, human primary explant cultures of both bone entities from ten donors were established. Subsequent transcription analysis detected higher gene expression of OPN in iHOBs compared to aHOBs, thereby confirming the results of our previous study. This difference was particularly apparent when cultures were derived from female donors. Hence, OPN protein expression as well as the angiogenic potential of OPN was analyzed, originating from HOBs of one female donor. In accordance to the gene expression level, secreted OPN was more abundant in the supernatant of iHOBs than in aHOBs. Moreover, secreted OPN was found to stimulate migration of HUVECs, but not proliferation or tube formation. These results indicate an involvement in very early stages of angiogenesis and a functional distinction of OPN from HOBs derived from different bone entities.

Purpose: An ongoing clinical trial regarding intra- and post-surgical morbidity in maxillary apicoectomies showed significant higher morbidity for upper canines and palatal roots of upper 1st premolars. Analysis of available presurgical cone beam computed tomography (CBCT)-scans revealed the existence of an unknown bone-canal branching off from the bone-canal or groove of the anterior superior alveolar artery (asaa). Aim of the study was the determination of the contents of this newly found bone canal in human cadaver heads, its prevalence as possible standard anatomical structure and its automatized detection with a contemporary high-resolution TRIUM-CBCT-device in vivo., Methods: 35 human cadaver heads were dissected, the prevalence of the bone-canal determined and its contents analyzed by histology. 835 consecutive routine high-resolution TRIUM-CBCT-scans from routine patients were analyzed by an automatized detection- and tracing-algorithm for in vivo-determination of prevalence of this bone canal. Automatized detection and additional manual tracing were statistically evaluated by SSPS 20.0 software., Results: The bone-canal was found in 96% of the anatomical specimens, its content identified as artery not described until now and named after the first finder "Arteria Kurrekii". Automatized tracing of TRIUM-CBCT-scans with additional manual tracing revealed an in vivo prevalence of this newly found artery of 95% (p ≤ 0.05)., Conclusions: The newly found anterior superior palatal alveolar artery (aspaa-"Arteria Kurrekii") might have the same clinical impact for surgical procedures in the maxilla as the posterior superior alveolar artery (psaa). Its first detection was enabled by high-resolution TRIUM-CBCT devices and prevalence as standard anatomical structure proven in vivo by automatized CBCT-scan analysis.

Purpose: The aim was to assess the effect of a relevant regimen of zoledronic acid (ZA) treatment for the study of bisphosphonate-related osteonecrosis of the jaw on alveolar bone microstructure and vasculature. A sub-objective was to use 3-dimensional imaging to describe site-specific changes induced by ZA in the alveolar bone., Materials and Methods: Five Wistar rats received ZA (0.6 mg/kg) and five (controls) received saline solution in the same volume. The compounds were administered intraperitoneally in 5 doses every 28 days. The rats were euthanized 150 days after therapy onset. The mandibles were scanned using high-resolution (14-μm) micro-computed tomography (micro-CT), decalcified, cut into slices for histologic analysis (5 μm), and stained with hematoxylin-eosin. Bone quality parameters were calculated using CT-Analyser software (Bruker, Kontich, Belgium) in 2 different volumes of interest (VOIs): the region between the first molar roots (VOI-1) and the periapical region under the first and second molars' apex (VOI-2). Blood vessel density and bone histomorphometric parameters were calculated only for the region between the roots of the first molar using AxioVision Imaging software (version 4.8; Carl Zeiss, Gottingen, Germany)., Results: ZA-treated rats showed a significant increase in percentage of bone volume and density (P < .05), with thicker and more connected trabeculae. Furthermore, the ZA group showed a significant decrease in the size of the marrow spaces and nutritive canals and in blood vessel density (P < .05). In the micro-CT evaluation, VOI-2 showed better outcomes in measuring the effect of ZA on alveolar bone., Conclusions: ZA treatment induced bone corticalization and decreased alveolar bone vascularization. VOI-2 should be preferred for micro-CT evaluation of the effect of bisphosphonates on alveolar bone. This analysis allowed the effect of ZA on alveolar bone and its vascularization to be characterized. Florid cemento-osseous dysplasia (FLCOD) is a rare, extensive bone metabolism disorder, which occurs only in the jaw bone. It is usually asymptomatic for a long time and discovered incidentally during a radiological examination. The characteristics of FLCOD in the initial stages are similar to those of periapical granuloma or jaw cyst, which may lead to misdiagnosis. After the lesion is mature, the imaging findings show that radiopaque with a thin radiolucent peripheral halo, which is crucial for the diagnosis of FLCOD, but other jaw lesions have similar imaging findings. Due to the poor blood supply of the lesion, the alveolar bone of root apices of vital teeth is slow to heal after trauma, increasing the chance of infection, which can lead to the osteomyelitis of the jaws and emerge sequestrum. This paper reviews the aspects of pathogenesis, clinical characteristics, diagnosis, differential diagnosis and treatment.

Purpose: The aim of this study was to investigate the protection of the inferior alveolar neurovascular bundle in alveolar bone operation in conditions such as chronic osteomyelitis and cementoma., Patients and Methods: The study enrolled 7 cases, 4 with chronic osteomyelitis and 3 with cementoma with pain. Computed tomography scan and 3-dimensional reconstruction were performed for the diseases. Data were processed by ProPlan CMF 1.3 software. The edge of lesion was defined and the inferior alveolar nerve was marked. Template was designed to guide the osteotomy line. Piezosurgery was used for osteotomy, with the avoidance of nerve canal. Current perception threshold (CPT) was performed to evaluate the nerve function after operation., Results: The CPT difference of the affected side before and after operation showed no statistically significant differences compared with that of the unaffected side (P = 0.0556)., Conclusions: Digital template protects the inferior alveolar neurovascular bundle with the aid of piezosurgery during alveolar bone resection, which obtained satisfying clinical results. As powerful assistive tools of functional surgery, digital template and piezosurgery achieve both the purposes of treatment and function.

It reports the results of primary basal blood flow in partially edentulous bone tissue with atrophy, as well as comparative evaluation of haemomicrocirculation level of the alveolar bone mucosa after bone augmentation via tunneling and conventional methods. The results of the initial state of microcirculation in the mucous membrane of the alveolar process in the partially edentulous area show a 45% decrease of blood flow (M), its intensity (s) by 60%. Microcirculatory shifts in blood flow levels indicators, its intensity, vasomotor activity are more evident when using the conventional method, where revascularization takes more than five months. When using the tunnel method, there is an improvement in haemodynamic mechanisms of tissue blood flow regulation. Microcirculation is restored 4 months after surgery.

Purpose: Sinus floor augmentation is a common procedure in implant dentistry. However, several intraoperative complications can occur during this procedure, such as bleeding from the lateral wall of the maxillary sinus. The aim of this study was to describe the vascular structures of the lateral wall of the maxillary sinus using a vascular labeling technique., Materials and Methods: Ten cadaveric specimens were prepared by the vascular labeling technique. Liquid latex was injected into the large vessels of the head, and the lateral wall of the maxillary sinus was exposed by dissection. The diameter of the vessels and their distance from the alveolar ridge (AR) were recorded., Results: Blood vessels could be observed in all the dissected specimens (100%). The mean distance from the lower edge of the blood vessels to the AR was 18.5 mm (SD 3.31 mm)., Conclusions: The vascular labeling technique detects maxillary sinus vessels in a predictable and effective way. These structures are clinically relevant because they are located in the area where the lateral window is usually created in sinus augmentation procedures and can cause profuse bleeding.

The purpose of this study was to evaluate the effects of segmental osteotomy on the blood vessels and osteoclasts in rats using micro-computed tomography (micro-CT) and histomorphometric analysis. After segmental osteotomy was performed around the maxillary first molars of 36 male Sprague-Dawley rats (n = 72), the samples were divided into a control group (no displacement), 0.5 D group (0.5 mm buccal displacement) and 1.0 D group (1.0 mm buccal displacement) (n = 24/group). At 1, 2, 4 and 8 weeks after surgery, changes in the blood vessel volume were investigated using micro-CT with perfusion of radiopaque silicone rubber. Tartrate-resistant acid phosphatase (TRAP) staining was used for histomorphometric analysis. Two-way repeated measures analysis of variance (rmANOVA) was performed to compare the volume of blood vessels and number of TRAP-positive osteoclasts among the groups. Regarding blood vessel volume, the displacement groups had no significant effects, while the time points had significant effects (p = 0.014). The blood vessel volume at 1 week was significantly smaller than that at 2, 4, and 8 weeks (p = 0.004, p = 0.026, and p = 0.005, respectively). Regarding TRAP cell count, the displacement groups had no significant effects, while the time points had significant effects (p < 0.001). The number of TRAP-positive osteoclasts at 8 weeks was significantly smaller than that at 1, 2, and 4 weeks (p < 0.001, p < 0.001, and p = 0.002, respectively), and the count at 4 weeks was smaller than that at 1 week (p = 0.011). Therefore, a regional osteoclast-related acceleratory phenomenon was maintained until 4 weeks after surgery.

Objectives: The aim of this anatomical study was to measure the distance from the intraosseous and extraosseous arterial anastomoses of the maxillary sinus to the alveolar ridge in edentulous specimens dissected from human cadavers., Materials and Methods: Forty edentulous maxillary specimens were dissected, anatomically prepared, and injected for a better visualization of the maxillary sinus artery., Results: This study showed that the mean distance from the alveolar ridge to the extraosseous anastomosis was 16 mm for the second maxillary molar, 12 mm for the first maxillary molar, and 13 mm for the second maxillary premolar. For the intraosseous anastomosis to the alveolar ridge, the mean distance was 17 mm for the second maxillary molar, 13 mm for the first maxillary molar, and 14 mm for the second maxillary premolar., Conclusions: Under the conditions of this study, we found that the mean distance from the alveolar crest to the extraosseous anastomosis and intraosseous anastomosis was shortest for the first maxillary molar and second maxillary premolar and greatest for the second maxillary molar., Clinical Relevance: These findings provide data whose relevance for clinical dentistry is that they can be applied to minimize the risk of damage to the arterial network of the maxillary sinus during surgical procedures in this region.

Background: The posterior superior alveolar (PSA) artery is frequently encountered in the area where the lateral osteotomy is performed during direct sinus augmentation procedures., Objective: To investigate the correlation between patient-dependent variables and measurements related to PSA using cone beam computed tomography (CBCT) data., Methods: Three hundred ninety-four CBCT scans were evaluated to assess the PSA artery diameter and distances to the sinus floor and to alveolar crest. Patient's age, gender, and edentulism status were recorded., Results: The PSA artery tends to be wider in older patients. Distances to the sinus floor or the alveolar crest tend to be shorter in women and in partially and completely edentulous patients. Also, as those distances decrease, the mediolateral width of the sinus increases., Conclusion: Tooth loss leads to maxillary sinus vertical collapse with respect to the PSA artery. The position of the artery is stable; so, the mediolateral dimensions at different heights from the floor increase., Practical Implications: The reduced distances from the PSA to the sinus floor and the alveolar crest in edentulous patients potentially increase the risk of injury during maxillary sinus lift. Additionally, when the distance to the sinus floor decreases, the mediolateral dimensions of the sinus at different heights increase, which may complicate the technique and challenge the outcomes.

There are no available detailed data on the three-dimensional courses of the human superior alveolar nerves and vessels. This study aimed to clarify the relationships of the maxillary sinus with the superior alveolar nerves and vessels using cone-beam computed tomography (CT) combined with μ-CT and histological analyses. Digital imaging and communication in medicine data obtained from the scanned heads/maxillae of cadavers used for undergraduate/postgraduate dissection practice and skulls using cone-beam CT were reconstructed into three-dimensional (3D) images using software. The 3D images were compared with μ-CT images and histological sections. Cone-beam CT clarified the relationships of the maxillary sinus with the superior alveolar canals/grooves. The main anterior superior alveolar canal/groove ran anteriorly through the upper part of the sinus and terminated at the bottom of the nasal cavity near the piriform aperture. The main middle alveolar canal ran downward from the upper part of the sinus to ultimately join the anterior one. The main posterior alveolar canal ran through the lateral lower part of the sinus and communicated with the anterior one. Histological analyses demonstrated the existence of nerves and vessels in these canals/grooves, and the quantities of these structures varied across each canal/groove. Furthermore, the superior dental nerve plexus exhibited a network that was located horizontally to the occlusal plane, although these nerve plexuses appeared to be the vertical network that is described in most textbooks. In conclusion, cone-beam CT is suggested to be a useful method for clarifying the superior alveolar canals/grooves including the nerves and vessels., (© 2016 Wiley Periodicals, Inc.)

Removal of third molars is a common surgical procedure with low complication rates. Localized alveolar osteitis, infection, bleeding, and paresthesia are the four most common postoperative complications of third molar extraction reported in the literature. Postoperative severe hemorrhages are rare and are usually related to inferior alveolar artery damage. Although most bleeding is usually managed effectively by local compression or packing of the socket, even life-threatening complications may occur. Endovascular embolization has been rarely reported as treatment for such a complication and represents an ideal solution, with a low complication rate and excellent control of bleeding. The authors report a case of potentially life-threatening hemorrhage continuing 4 days after extraction of the mandibular right third molar, resulting in significant anemia (Hb 6.6 g/dL) and treated successfully with endovascular embolization of the inferior alveolar artery. The authors consider it important for general practitioners to know this treatment and how to manage this rare complication in the correct way, saving time if satisfactory hemostasis cannot be reached with common procedures.

The purpose of this study was to examine the diameter, location, and frequency of the appearance of the posterior superior alveolar artery (PSAA) in preoperative cone beam computed tomography (CBCT) scans. Two hundred and eleven pre-implant CBCT scans were reviewed. The following criteria were considered in all subjects: (1) the location of the artery: intra-sinus or below the membrane (type I), intraosseous (type II), or superficial (type III); (2) the distance between the lower border of the artery and the alveolar crest; (3) the bone height measured from the floor of the sinus to the crest of the ridge; (4) the distance from the lateral wall of the artery to the medial wall of the maxillary sinus; and (5) the diameter of the artery (in millimetres). The distance between the artery and the medial sinus wall, as well as the diameter of the artery, were greater in patients with an alveolar bone height ≤10mm than in those with a bone height >10mm. The distance from the artery to the medial sinus wall and the diameter of the artery were positively correlated with the number of missing teeth. A systematic search of MEDLINE, Embase, and Proceedings Web of Science was undertaken to assess the prevalence of the posterior superior alveolar artery (PSAA) in the lateral sinus wall in sinus lift patients, as identified using computed tomography (CT)/cone beam computed tomography (CBCT). For inclusion, the article had to report PSAA detection in the bony wall using CT and/or CBCT in patients with subsinus edentulism. Studies on post-mortem findings, mixed samples (living and cadaveric), those presenting pooled results only, or studies performed for a sinus pathology were excluded. Heterogeneity was checked using an adapted version of the DerSimonian and Laird Q test, and quantified by calculating the proportion of the total variance due to between-study variance (Ri statistic). Eight hundred and eleven single papers were reviewed and filtered according to the inclusion/exclusion criteria. Ten studies were selected (1647 patients and 2740 maxillary sinuses (study unit)). The pooled prevalence of PSAA was 62.02 (95% confidence interval (CI) 46.33-77.71). CBCT studies detected PSAA more frequently (78.12, 95% CI 61.25-94.98) than CT studies (51.19, 95% CI 42.33-60.05). Conventional CT revealed thicker arteries than CBCT. It is concluded that PSAA detection is more frequent when CBCT explorations are used. Additional comparative studies controlling for potential confounding factors are needed to ascertain the actual diagnostic value of radiographic explorations for assessing the PSAA prior to sinus floor elevation procedures., (Copyright © 2015 International Association of Oral and Maxillofacial Surgeons. The blood supply to both the lateral wall of the maxillary sinus and the overlying membrane originates from the posterosuperior alveolar artery (PSAA) and the infraorbital artery. The purpose of the present study was to evaluate the anatomic characteristics of the PSAA in a large number of subjects of the Lebanese population. Images of 696 sinuses were analyzed using cone beam computed tomography (CT). Coronal, axial, and sagittal CT images were evaluated for the presence of an osseous canal in the lateral wall of the sinus, and the prevalence, position, and location of the canal were studied and presented.

The aim of this study was to investigate the effect of changes in end-tidal carbon dioxide tension (ETCO2) during remifentanil (Remi) infusion on oral tissue blood flow in rabbits. Eight male tracheotomized Japan White rabbits were anesthetized with sevoflurane under mechanical ventilation. The infusion rate of Remi was 0.4 μg/kg/min. Carbon dioxide was added to the inspired gas to change the inspired CO2 tension to prevent changes in the ventilating condition. Observed variables were systolic blood pressure (SBP), diastolic blood pressure (DBP), mean arterial pressure (MAP), heart rate (HR), common carotid artery blood flow (CCBF), tongue mucosal blood flow (TBF), mandibular bone marrow tissue blood flow (BBF), masseter muscle tissue blood flow (MBF), upper alveolar tissue blood flow (UBF), and lower alveolar tissue blood flow (LBF). The CCBF, TBF, BBF, UBF, and LBF values were increased, while MBF was decreased, under hypercapnia, and vice versa. The BBF, UBF, and LBF values were increased, while the MBF value was decreased, under hypercapnia during Remi infusion, and vice versa. The BBF, MBF, UBF, and LBF values, but not the CCBF and TBF values, changed along with ETCO2 changes during Remi infusion.

Objectives: We investigated the effect of soft tissue expansion (STE) on vertical ridge augmentation with regard to the incidence of wound dehiscences and the impairment of microcirculation in dogs, and the applicability of laser Doppler flowmetry (LDF) to explore the relation between microcirculation and wound healing., Material and Methods: Bone defects were created on both mandibular sides in ten beagle dogs by extraction of premolars and removal of bone. Six weeks later, self-filling tissue expanders were implanted in randomly assigned test sites. After 5 weeks of expansion, vertical augmentation was carried out in test and control sites using calvarial onlay grafts side by side with granular biphasic calcium phosphate covered with a resorbable polyethylene glycol membrane. Microcirculation was evaluated with laser Doppler flowmetry (LDF). The incidence of wound dehiscences was evaluated after 2 weeks. The validity of LDF to predict dehiscences was evaluated by construction of receiver operating characteristic (ROC) curves., Results: After augmentation, test sites showed significantly better perfusion than control sites without preceding STE (P = 0.012). Three days after surgery, perfusion was still significantly decreased in control sites (P = 0.005), while microcirculation in test sites had returned to pre-surgical levels. After 2 weeks, healing in test sites was good, whereas eight dehiscences were found in control sites (P = 0.002). ROC curves showed that microcirculation levels immediately after augmentation surgery significantly predicted subsequent wound dehiscences (AUC = 0.799, CI 0.642-0.955, P = 0.006)., Conclusions: Laser Doppler flowmetry is suitable for evaluation of soft tissue microcirculation after ridge augmentation. A decrease in arterial carbon dioxide tension induces an increase in masseter muscle blood flow and a decrease in mandibular bone marrow blood flow during general anesthesia. In addition, dexmedetomidine infusion reduces oral tissue blood flow. In this study we investigated how end-tidal carbon dioxide tension (ET-CO2 ) changes influence on oral tissue blood flow during continuous dexmedetomidine infusion in rabbits. Eleven male Japan White rabbits were anesthetized with sevoflurane. Then, ET-CO2 was set at 30 mmHg and adjusted to 40 and 60 mmHg, and heart rate, systolic blood pressure, diastolic blood pressure, mean arterial pressure, common carotid artery blood flow, mandibular bone marrow blood flow, masseter muscle blood flow, and blood flow in other oral tissues were measured. Following this, the ET-CO2 was returned to 30 mmHg and dexmedetomidine was infused over 60 min. The measurements were repeated. Most parameters increased, regardless of whether or not dexmedetomidine was present, and heart rate and masseter muscle blood flow decreased in an ET-CO2 -dependent manner. Dexmedetomidine infusion suppressed ET-CO2 -dependent masseter muscle blood flow change. Masseter muscle blood flow during ET-CO2 at 30 mmHg with dexmedetomidine was the same as that during ET-CO2 at 40 mmHg without dexmedetomidine. Our findings suggest that dexmedetomidine infusion and slight hypocapnia under general anesthesia suppress an increase in masseter muscle blood flow as well as reducing mandibular bone marrow blood flow. Objectives: Mental artery flow decreases with age and may have an aetiological role in alveolar ridge atrophy. The aim of this study was to identify factors associated with alterations of mental artery flow, assessed by ultrasonography., Methods: This case-control study was conducted on elderly patients (aged above 60 years) at the beginning of dental treatment. Intraoral B-mode Doppler ultrasonography was used to assess mental artery flow. The cases were defined as patients with a weak/absent ultrasound signal, whereas the controls presented a strong ultrasound signal. Demographics and radiographic findings (low bone mineral density on dual-energy X-ray absorptiometry and mandibular cortical index on panoramic radiographs) were analysed as risk factors for weak/absent ultrasound signal and were calculated as adjusted odds ratios (AORs) with 95% confidence intervals (CIs) using conditional logistic regression. In addition, the Student's t-test was used to compare the mean alveolar bone height of the analysed groups. A p-value <0.05 was considered statistically significant., Results: A total of 30 ultrasound examinations (12 cases and 18 controls) were analysed. A weak/absent mental artery pulse strength was significantly associated with edentulism (AOR = 3.67; 95% CI = 0.86-15.63; p = 0.046). In addition, there was a significant difference in alveolar bone height between edentulous cases and controls (p = 0.036)., Conclusions: Within the limitations of this study, the present results indicate that edentulism is associated with diminished mental artery flow, which, in turn, affects alveolar bone height.

Introduction: Bisphosphonate-related osteonecrosis of the jaws (BRONJ) is a pathologic condition of increasing frequency, with a poorly understood pathophysiology and which can be difficult to manage. The aim of this study was to find a reproducible experimental model that directly relates chronic bisphosphonate administration with the development of osteonecrosis with or without tooth extraction, with no other drug involved., Material and Methods: Twenty male Wistar rats were divided into 4 groups (n = 5/group). Animals were injected over 9 weeks with zoledronic acid (0.1 mg/kg). In groups 1 and 2 three times a week intraperitoneally, and in group 3 once a week intravenously. A control group (group 4) received intraperitoneal injections of saline solution three times a week. After 8 weeks of treatment, 3 right upper jaw molars were extracted in groups 1, 3 and 4 and all rats were sacrificed 1 week later. The maxillae were histologically analyzed for presence of osteonecrosis foci, number of osteoclasts, vascularity, bone resorption status and presence of abscess. Radiographic examination was performed with a plain radiograph of each hemi-head., Results: We found that group 1 (dental extractions and highest cumulative dose of zoledronic acid) had the highest incidence of osteonecrosis (80%), absence of bone resorption (100%) and lowest number of osteoclasts (mean 7.9/field at 40×). Zoledronic acid-treated groups showed variable degrees of osteosclerosis and trabecular disorganization on X-ray study., Conclusions: We offer a new animal model of BRONJ after zoledronic acid administration and dental extractions, achieving bone changes similar or superior to previous studies, highlighting the dental extraction as an important trigger factor., (Copyright © 2013 European Association for Cranio-Maxillo-Facial Surgery. Introduction: Nicotine is harmful to angiogenesis, osteogenesis and synthesis of collagen., Objective: The aim of this study was to investigate the effect of nicotine on bone remodeling during orthodontic movement in rats., Methods: Eighty male Wistar rats were randomly divided into three groups: Group C (control), group CM (with orthodontic movement) and group NM (nicotine with orthodontic movement) groups. The animals comprising groups C and CM received 0.9% saline solution while group NM received nicotine solution (2 mg/kg). A nickel-titanium closed-coil spring was used to induce tooth movement. The animals were euthanized and tissue specimens were histologically processed. Blood vessels, Howship's lacunae and osteoclast-like cells present in the tension and compression areas of periodontal ligaments were quantified. The extent of bone formation was evaluated under polarized light, to determine the percentage of immature/mature collagen., Results: It was observed lower blood vessel densities in the NM group in comparison to the CM group, three (p < 0.001) and seven (p < 0.05) days after force application. Osteoclast-like cells and Howship's lacunae in the NM group presented lower levels of expression, in comparison to the CM group, with significant differences on day 7 (p < 0.05 for both variables) and day 14 (p < 0.05 for osteoclast-like cells and p < 0.01 for Howship's lacunae). The percentage of immature collagen was increased in the NM group in comparison to the CM group, with a statistically significant difference on day 3 (p < 0.05), day 7 (p < 0.001), day 14 (p < 0.001) and day 21 (p < 0.001)., Conclusions: Nicotine affects bone remodeling during orthodontic movement, reducing angiogenesis, osteoclast-like cells and Howship's lacunae, thereby delaying the collagen maturation process in new bone matrix.

To determine the altered location of the medial lingual nutritional foramina in different stages of bone resorption, 55 cadaveric mandibles were selected for this study from a total of 1532. Prevalence, location of the foramen (lingual, cranial, labial), application of Atwood classification (grades 1-6), and macroanatomic dissections were tools of this examination. An increasing grade of atrophy leads to a shift in position of the medial lingual nutritional foramen.

The case of a patient with high-flow arteriovenous malformations (AVMs) affecting the mandible and lower face is presented. The patient had undergone surgical resection, sclerotherapy, and transcatheter embolization over the course of 16 years. As a result, all teeth except for the lower left third molar were missing, and a mass of arteriovenous malformed soft tissue covered the residual alveolar ridge. Implant placement surgery was performed in his mandible after treatment of the AVMs. Osseointegration of the implants placed in the mandible affected by the AVMs was evaluated. Rehabilitation in the absence of the alveolar ridge or keratinized gingiva was also evaluated. The implant stability quotient (ISQ) was measured before fabricating the restoration. An implant-supported partial denture was placed 4 months postoperatively. No implant failure was found; no bleeding on probing was present. The occlusion and masticatory function were satisfactory at 2-year follow-up. Osseointegration was safely achieved in the bone affected by the intraosseous AVMs that had been embolized. Assessment of the maxillary sinus anatomy before sinus augmentation is important for avoiding surgical complications, because of the close anatomical relationship between the posterior maxillary teeth and the maxillary sinus. The posterior superior alveolar artery (PSAA) is the branch of the maxillary artery that supplies the lateral sinus wall and overlying membrane. We evaluated the location of the PSAA and its relationship to the alveolar ridge and maxillary sinus using cone beam computed tomography (CBCT). The study group consisted of 135 CBCT scans (270 sinuses) obtained from the archive of the dentomaxillofacial radiology department at Yeditepe University Faculty of Dentistry, Istanbul, Turkey. The distance between the lower border of the artery and the alveolar crest, bone height from the sinus floor to the ridge crest, distance from the artery to the medial sinus wall, and the diameter and location of the artery were determined. The occurrence of septa and pathology were recorded from CBCT scans. The PSAA was observed in 89.3% of sinuses, and 71.1% of arteries were intraosseous with diameters mostly < 1 mm (68.9%). The prevalence of sinus septa was 55.2%, and that of sinus pathology was 57.4%. The mean age was 43.07 ± 17.55 years. There was a statistically significant difference between the location of the artery and gender (p < 0.05). The prevalence of sinus membrane thickening was 57.4%. Detailed knowledge about the location of the PSAA and sinus morphology may be obtained with CBCT before maxillary sinus surgery.

Objective: The purpose of this paper was to find a correlation between laser doppler flowmetry (LDF) and the bone mineral density quotient (BMDQ) to evaluate irradiated bone quality preoperatively., Materials and Methods: Premolars and molars were extracted from six minipigs. After a three-month healing period, three animals received irradiation at a total dose of 24 Gy. Three months after irradiation, quantitative computed tomography was performed using a calibration bone phantom to determine the 120-position BMDQ in the alveolar bone. A drill template was created to define the exact location for measurement. LDF was then recorded after osteotomy of the residual alveolar ridge. The correlation between the BMDQ and LDF was investigated., Results: There was a linear negative correlation between LDF and BMDQ in non-irradiated bone (r = -0.435, P = 0.001) and less pronounced also in irradiated bone (r = -0.309, P = 0.017). In both non-irradiated and irradiated bone, we found distinct differences between the maxilla and mandible with respect to BMDQ. However, a clear difference in mandibular and maxillary vascularity was only seen in non-irradiated bone., Conclusion: In non-irradiated bone, LDF and BMDQ were inversely correlated. In irradiated bone, the BMDQ alone is not an adequate preoperative tool for evaluating bone quality because it was not correlated with bone perfusion., (© 2012 John Wiley & Sons A/S.)

Intraoral bone grafting is routinely employed for implant site development. Bone graft consolidation is a complex biologic process depending on the formation of blood vessels into the augmented area. Hypoxia-inducible factors (HIFs) and hypoxia-mimicking agents (HMAs) are key stimulators of blood vessel formation. Hypoxia prevents HIFs from degradation, thus signaling angiogenesis. Under normoxia, HMAs prevent degradation of HIFs. The cellular and molecular mechanisms responsible for angiogenic-osteogenic coupling and the therapeutic manipulation of HIFs and HMAs in intraoral bone repair and regeneration are discussed. Such discoveries suggest promising approaches for the development of novel therapies to improve intraoral bone repair and regeneration procedures.

Objective: In this work we evaluated the bone-forming potential of BMP4, TGFβ1 and BMP4/TGFβ1 mixed by performing histological and morphometric analysis. We also evaluated the immunolabelling of fibronectin (FN) and collagen type III (Col III), two determinant proteins for the early phase of bone repair., Design: Histological, histomorphometric and immunohistochemistry analysis were used to evaluate new bone and blood vessels formation as well as fibronectin and collagen type III expression. 112 male Wistar rats weighing 250-300g had their maxillary second molar extracted. Sockets filled with blood clot (BC) or treated with L (empty liposome), P (PBS), BP (BMP-4 in PBS) and TP (TGF-β1 in PBS), as well as with BL (BMP-4 in liposome) and TL (TGF-β1 in liposome) administered isolated or in association (BTL) were obtained. The animals were sacrificed at 3, 7, 14 and 21 days after surgery., Results: An increased percentage of bone trabeculae, and a higher number of blood vessels were observed in groups BL or TL administered isolated or in association when compared to groups BC, L, P, BP and TP. Fibronectin and collagen type III analysis revealed enhanced expression firstly detected at 3 days followed by a peak at 7 days. Lower levels of immunoreactivity were observed in the sockets filled with blood clot, and treated with L, P, BP and TP when compared with sockets from groups BL, TL and BTL., Conclusion: The present study indicates growth factors carried by liposomes, either in isolated or associated forms, as successful enhancers of the healing process in rat tooth sockets. Background: Arteriovenous malformations (AVM) of the head and neck are rare conditions with the majority of primary sites in the mid-face. Progression can lead to massive life-threatening bleeding. Treatment of choice is the combination of embolization and surgical resection. Diagnosis and management of AVM has been facilitated in recent years by the progress in imaging techniques and various microsurgical and endovascular embolization procedures., Methods: This report presents the interdisciplinary approach to a case of life-threatening AVM of the mid-face in a 30-year-old patient., Results: Angiography with embolization followed by resection and plastic reconstruction was performed. Follow-up showed incomplete obliteration of the AVM and lead to additional embolizations via the anastomosed radial artery. Complete eradication of the AVM was not possible during the three year follow-up period., Conclusion: In extensive vascular malformations of the head and neck, complete surgical removal is often impossible and associated with high risk of complications and mortality. Among alveolar ridge augmentation techniques, the ridge-split procedure demonstrates many benefits, including no need for a second (donor) surgical site, rare risk of inferior alveolar nerve injury, and less pain and swelling, and others. Lateral bone augmentation through the ridge-split works best in a localized lateral bony defect intended for 1 or 2 implants and where the ridge is vertically intact. In this article, the authors present a detailed description of the implant-driven technique of alveolar ridge-split procedure in small and large bone deficiencies, in maxilla and mandible, supplemented by multiple photographs. The authors emphasize the need for careful manipulation of the thin ridge based on knowledge of precise surgical principles and stress that a practitioner needs specialized training and experience to perform this type of alveolar bone augmentation.

Arteriovenous malformation of the mandible is a rare and life-threatening lesion. The gold standard treatment is to extirpate the lesion surgically after endovascular embolization. Preoperative endovascular embolization is widely accepted, but there are limitations to the treatment. The authors introduce a technique in which the lesion is divided into parts and filled with bone wax, then curetted step by step. Particle embolization for epistaxis and intra-oral hemorrhage are performed on an as needed basis by neurointerventionalists. A case is presented of massive oral hemorrhage associated with end stage liver disease coagulopathy after tooth extraction of bilateral maxillary and mandibular molars. A man in his fifties with end stage liver disease who presented for evaluation of a syncopal episode was determined to be in hemorrhagic shock from 2 days of persistent oral bleeding after elective tooth extractions. Conservative management with multiple blood transfusion products, packing and vasoconstrictive spray was ineffective. Microcatheter angiograms of the alveolar arteries demonstrated blood and contrast pooling within the sockets of the extracted teeth. Selection of the bilateral supplying alveolar and inferior alveolar artery branches was achieved followed by polyvinyl alcohol particle embolization (250-355 μm). Polyvinyl alcohol particle embolization of dental socket hemorrhages is technically feasible and effective.

Ethnopharmacological Relevance: Dipsacus asper Wall (Dipsacaceae), Salvia miltiorrhiza (Salvia) and Drynaria fortunei (Drynaria) have been traditionally used in Chinese medicine as the main ingredient of many formulations for the treatment of cardiovascular and inflammatory diseases., Aim of the Study: This study was aimed to evaluate the effect of the traditional Chinese medicine (TCM) Dipsacaceae, Salvia and Drynaria on bone remodeling during orthodontic tooth movement (OTM)., Setting: This study was conducted in School of Stomatology, Shandong University between 2009 and 2010 [Jinan, Shandong, China]., Materials and Methods: Ninety-six eight-week-old female SPF Wistar rats 180-200 g were selected and randomly divided into four groups of 24: Dipsacaceae group, Salvia group, Drynaria group and control group. Animal models for orthodontic tooth movement were then established which consisted of a closed coil spring ligated to the upper first molar and incisors, exerting a force of 40 g during the experimental period. Rats in the TCM groups were given Dipsacaceae, Salvia and Drynaria decoction respectively by intragastric administration 6 g/kg/day and the control group were given normal saline 3 ml. The rats were sacrificed in batch on the 7th, 14th, 21st and 28th days after orthodontic treatment. Slices from periodontium of the upper first molar were observed under optical microscope. Neovascularization, new bone formation and osteoclast number were observed., Results: The upper first molars were drawn mesial by the force. Telangiectasia and new bone formation in periodontal tissue were significantly in the TCM groups compared with the control group. Application of orthodontic forces in the experimental teeth showed a significant increase (P<0.05) of osteoclast number in the TCM group when compared with the control group. In addition, the number of osteoclast had no significant differences among the TCM groups (P>0.05). Osteoclast number in the TCM group and the control group were 10.12±0.058, 10.13±0.022, 10.09±0.047 and 9.55±0.045, respectively., Conclusions: These findings suggest that the TCM decoction are beneficial to the alveolar bone remodeling by promoting osteoclast differentiation during OTM., (Copyright © 2011 Elsevier Ireland Ltd. Context: It was suggested that the accessory neurovascular foramina of the mandible might be of significance in relation to the effectiveness of local anesthesia following the routine inferior alveolar nerve block., Aims: To investigate the incidence of neurovascular foramina over the lingual surface of the mandible in South Indian population., Settings and Design: The study was conducted at the department of anatomy., Materials and Methods: The study included 67 human adult dry mandibles, the exact ages and sexes of which were not known. The location and number of neurovascular foramina were topographically analyzed., Statistical Analysis Used: Descriptive statistics., Results: The foramina were observed in 64 mandibles (95.5%) and were often multiple in most of the cases. They were located between the two medial incisors in 8 mandibles (1.9%), between the medial and lateral incisor in 34 mandibles (50.7%; 25-bilateral; 7-right; 2-left), between the lateral incisor and canine in 7 mandibles (10.4%; 2-bilateral; 3-right; 2-left), between the canine and first premolar in 6 cases (8.9%; 3 on each side). Foramina were also present around the genial tubercle in 56 mandibles (83.6%). Among them, 52 mandibles showed a single foramen just above the genial tubercle, 34 mandibles had foramina below the tubercles, 13 mandibles had foramina on the right side of genial tubercle and 17 were having on the left side., Conclusion: Since the anatomical details of these foramina are important to various fields of dentistry and oncology, the present investigation was undertaken. The clinical significance and implications are emphasized.

Objectives: The purpose of this study was to investigate the effects of naloxone and phentolamine on the blood flow changes in rabbit oral tissue induced by remifentanil during sevoflurane anesthesia., Materials and Methods: Male Japan White rabbits were anesthetized with sevoflurane under mechanical ventilation. Remifentanil was continuously infused at a rate of 0.4 μg/kg/min. Naloxone 0.01 mg/kg or phentolamine 0.01 mg/kg was administered during remifentanil infusion. Observed variables were systolic and diastolic blood pressures, mean arterial pressure, heart rate, common carotid artery blood flow, tongue mucosal blood flow, mandibular bone marrow blood flow, masseter muscle blood flow, and upper and lower alveolar tissue blood flows. The common carotid artery blood flow was monitored continuously using an ultrasonic blood flowmeter. Tongue mucosal blood flow was monitored continuously using a laser Doppler blood flowmeter. Mandibular bone marrow blood flow, masseter muscle blood flow, and upper and lower alveolar tissue blood flows were measured using a hydrogen clearance tissue blood flowmeter. One-way analysis of variance for repeated measurements followed by the Student-Newman-Keuls test was used., Results: Remifentanil produced decreases in the heart rate, systolic blood pressure, and common carotid artery blood flow by about 15% and mandibular bone marrow blood flow, masseter muscle blood flow, and upper and lower alveolar tissue blood flows by about 30%. In the naloxone group, all variables recovered after naloxone administration. In contrast, in the phentolamine group, tissue blood flow recovered, whereas heart rate, systolic blood pressure, and common carotid artery blood flow did not recover after phentolamine administration., Conclusions: Remifentanil deceased oral tissue blood flow and systemic hemodynamic variables. Objectives: Knowledge and evaluation of the maxillary sinus anatomy before sinus augmentation are essential for avoiding surgical complications. Posterior superior alveolar artery (PSAA) is the branch of maxillary artery that supplies lateral sinus wall and overlying membrane. The aims of this study were to examine the prevalence, diameter, and location of the PSAA and its relationship to the alveolar ridge and to study the prevalence of the sinus pathology and septum using computerized tomography (CT) scans., Materials and Methods: One hundred and twenty-one CT scans (242 sinuses) from patients undergoing sinus augmentation procedure and/or implant therapy were included. Lower border of the artery to the alveolar crest, bone height below the sinus floor to the ridge crest, distance of the artery to the medial sinus wall, diameter of the artery, and position of the artery were measured; presence of septa and pathology were recorded from CT sections., Results: Prevalence of sinus septa and sinus pathology was 16.1% and 24.8%, respectively. Artery was seen in 64.5% of all sinuses and was mostly intraosseous (68.2%). Mean diameter of PSAA was found 1.3 ± 0.5 mm. No significant correlation between the diameter of the artery and age was observed., Conclusions: The results from this study suggested that CT scan is a valuable tool in evaluating presence of sinus pathology, septa, and arteries before maxillary sinus surgery. Objectives: Impaired vascularization in the etiopathology of aminobisphosphonate-associated osteonecrosis of the jaw (BONJ) is assumed, but evidence is lacking. This immunohistochemical study differentiated vascularization and angiogenesis in BONJ-adjacent mucoperiosteal tissue., Study Design: Twenty BONJ (after zoledronate treatment) and 20 control mucoperiosteal tissue samples were processed with an autostaining-based alkaline phosphatase-antialkaline phosphatase staining kit. Vascularization was assessed by CD31 staining and angiogenesis-related neovessels by CD105 staining. The ratio of stained capillary area to total area of visible field was assessed. Statistics included Bonferroni adjustment., Results: CD31-stained microvessels were detected in each section and CD105-stained neovessels in each control. BONJ-adjacent mucoperiosteal tissue showed significantly fewer CD105-positive vessels in capillary areas (P < .05) than control samples. CD31-stained capillary area was not significantly reduced in mucoperiosteal BONJ-samples., Conclusions: Angiogenesis is impaired in BONJ-related mucoperiosteal tissue, but vascularization remains unaffected. Purpose: The aim of this study was to investigate the effect of remifentanil continuous infusion on the blood flow in rabbit oral tissue during propofol and sevoflurane anesthesia, as well as its dose dependency., Materials and Methods: Sixteen male tracheotomized Japan white rabbits were anesthetized with propofol or sevoflurane under mechanical ventilation. The infusion rate of remifentanil was 0.1, 0.2, 0.4, 0.8, and 1.6 μg·kg(-1)·min(-1), respectively, for 20 minutes. Observed variables were systolic blood pressure (SBP), diastolic blood pressure, mean arterial pressure (MAP), heart rate (HR), common carotid artery blood flow, tongue mucosal blood flow (TBF), mandibular bone marrow blood flow (BBF), masseter muscle blood flow (MBF), upper alveolar tissue blood flow (UBF), and lower alveolar tissue blood flow (LBF)., Results: HR, SBP, MAP, TBF, BBF, MBF, UBF, and LBF showed dose-dependent decreases during remifentanil infusion with both propofol and sevoflurane anesthesia. The decreases in BBF, MBF, UBF, and LBF explained as a percent change were greater than those in HR, SBP, MAP, and TBF. In the sevoflurane group, there were no changes in diastolic blood pressure and common carotid artery blood flow during remifentanil infusion., Conclusion: Our findings suggest that infusion of remifentanil reduces TBF, BBF, MBF, UBF, and LBF in a dose-dependent manner without a significant reduction in MAP during propofol and sevoflurane anesthesia., (Copyright © 2011 American Association of Oral and Maxillofacial Surgeons. Purpose: The purpose of this study was to elucidate the differences of the prevalence and diameter of the posterior superior alveolar artery (PSAA) and the distance of its inferior border from the alveolar crest on computed tomography (CT) images according to age and sex., Materials and Methods: CT images of maxilla in 200 patients were randomly selected from patients who underwent CT imaging at Yonsei University Dental Hospital, and analyzed. The prevalence of the PSAA in maxillary sinus and the distance of its inferior border from the alveolar crest in the premolar and molar area were measured., Results: The average prevalence of the PSAA on CT images was 52.0%, and it is higher in males (64%) than in females (40%). The diameter of the PSAA was 1.52 ± 0.47 mm (mean ± SD), and larger in males. The distance from the PSAA to the alveolar crest was greater in the premolar area (18.90 ± 4.21 mm) than in the molar area (15.45 ± 4.04 mm), and it did not differ significantly with age or sex., Conclusions: The prevalence of the PSAA on CT images was higher, and the diameter was larger in males. The PSAA was more close to the alveolar crest in the molar areas. The evaluation of the PSAA in maxillary sinus on CT images before surgery could reduce the likelihood of excess bleeding during surgery especially in molar areas.

Objectives: To investigate the prevalence, location, size and course of the anastomosis between the dental branch of the posterior superior alveolar artery (PSAA), known as alveolar antral artery (AAA), and the infraorbital artery (IOA)., Material and Methods: The first part of the study was performed on 30 maxillary sinuses deriving from 15 human cadaver heads. In order to visualize such anastomosis, the vascular network afferent to the sinus was injected with liquid latex mixed with green India ink through the external carotid artery. The second part of the study consisted of 100 CT scans from patients scheduled for sinus lift surgery., Results: An anastomosis between the AAA and the IOA was found by dissection in the context of the sinus anterolateral wall in 100% of cases, while a well-defined bony canal was detected radiographically in 94 out of 200 sinuses (47% of cases). The mean vertical distance from the lowest point of this bony canal to the alveolar crest was 11.25 ± 2.99 mm (SD) in maxillae examined by CT. The canal diameter was <1 mm in 55.3% of cases, 1-2 mm in 40.4% of cases and 2-3 mm in 4.3% of cases. In 100% of cases, the AAA was found to be partially intra-osseous, that is between the Schneiderian membrane and the lateral bony wall of the sinus, in the area selected for sinus antrostomy., Conclusions: A sound knowledge of the maxillary sinus vascular anatomy and its careful analysis by CT scan is essential to prevent complications during surgical interventions involving this region., (© 2010 John Wiley & Sons A/S.)

Bone modeling and remodeling following tooth extraction has been studied extensively. The reason for bone loss during the remodeling process is multifactorial, and the primary reason for this loss is still yet to be determined. The aim of this study was to examine the type of bone and the blood supply to the buccal bone in monkeys. Six maxillary arches from six monkeys were used. The arches were divided into three sections: right posterior, anterior from canine to canine, and left posterior. Blocks were decalcified and prepared for histologic processing and examination. Modified Masson trichrome and retic staining were used. Histologic sectioning demonstrated that the blood supply to the buccal bone came from the inner (socket) side of the alveolus, the periodontal ligament, the adjacent interdental bone, and the supraperiosteal vessels emanating from the covering gingiva or mucosa. Histologic examination showed that the buccal bone was composed of bundle and cortical bone. The thickness of the buccal bone was not uniform coronoapically, and the thinnest area of buccal bone was the coronal portion.

A thorough knowledge of the anatomy of the pterygomandibular space is essential for the successful administration of the inferior alveolar nerve block. In addition to the inferior alveolar and lingual nerves, other structures in this space are of particular significance for local anaesthesia, including the inferior alveolar vessels, the sphenomandibular ligament and the interpterygoid fascia. These structures can all potentially have an impact on the effectiveness of local anaesthesia in this area. Greater understanding of the nature and extent of variation in intraoral landmarks and underlying structures should lead to improved success rates, and provide safer and more effective anaesthesia. Background: Recent studies in animals have shown pronounced resorption of buccal bone plate after immediate implantation. The sectioning of experimental material for histologic evaluation of the bone plates could provide valuable information about the possible effect of bone exposure in periodontal and implant surgeries., Methods: Twenty-four incisors were collected from dogs. After decalcification, the blocks were immersed in paraffin and bucco-lingual histologic sections were examined under light microscope. Some sections were reserved for immunohistochemical analysis., Results: The bone density, the width of the bone plates, and the percentage of vessels presented in the periodontal ligament and periosteum were analyzed in the buccal and lingual bone plates, which were divided corono-apically into thirds. The buccal bone plates showed statistically higher bone density compared to the lingual bone plates in the coronal thirds. The width of both bone plates increased from the coronal to the apical third, but all the buccal thirds were significantly thinner compared to the lingual thirds. No statistically significant differences were found between the bone plates for the percentage of area occupied by the blood vessels in the periodontal ligament or periosteum., Conclusion: It is reasonable to conclude that the higher bone density, represented by the lower number of marrow spaces, in association with the thinner aspect of the buccal bone plates made them more fragile to absorb compared to the lingual bone plates, especially during mucoperiosteal procedures.

Background: Lingual vascular canal (LVC) is an important anatomical structure in mandibular anterior region. Trauma to this structure during implant placement has been reported in this study. Dental computed tomography (DCT) provides a three-dimensional visualization of lingual vascular canal., Aim: To assess the frequency, location, and size of LVC using dental CT., Materials and Methods: A retrospective analysis of 75 mandibular DCT was done. Evaluation was done to detect the frequency, size, and number of lingual vascular canal using Seimens Somatom Sensation 64., Results: About 73.3% patients (male=34, female=21) demonstrated presence of LVC with only one patient showing two canals. The mean distance from the inferior border of mandible was 0.5 mm, S.D.±0.70. The mean diameter of the canal was 0.31 mm, S.D.±0.70., Conclusion: Dental CT provides adequate information regarding frequency, number, and size of lingual vascular canal, which is an important anatomical structure in mandibular anterior region.

Studying of microhemodynamic processes in tissues of a prosthetic bed, according to LDF, has shown stimulating influence of rational orthopedic treatment on a microcirculation condition.

Cystic angiomatosis of bone is a rare condition of multifocal angiomas of the skeleton. The condition is believed to be congenital, grows slowly and starts in first decades of life. Two cases of progressive bimaxillary enlargement, presented here with a history of slowly enlargement of facial bones when they were 9 and 6 year old, respectively. Radiographic evaluation of the craniofacial bones revealed aggressive hypertrophy with severe displacement of the teeth. The histopathological evaluation of the gross specimen showed vital bone containing capillary and cavernous spaces with endothelial lining.
