The purpose of the work is to investigate the range of renoprotection, in particular osmotic and volumoregulatory functions, with combined application of sorbitol and L-arginine in patients with endogenous intoxication syndrome of purulent-septic genesis in the period of development of secondary toxic autoagression. Materials and methods. The indices of volumо-, and osmoregulatory functions of the kidneys in three groups of patients were investigated. The first group (control) included 31 patients with systemic inflammatory response syndrome (SIRS). The second group consisted of 27 patients with endogenous intoxication syndrome (EIS) who received standard therapy. The third group consisted of 29 patients with EIS, which on the basis of standard therapy were administered sorbilact. The fourth group included 30 patients with EIS, which on the basis of standard treatment were administered a combination of sorbilact and L-arginine. Infusion of sorbilact for patients of Groups III and IV was carried out at a rate of 6-7 ml/kg bodyweight intravenously at a rate of 7-8 ml/min. After the end of infusion of sorbilact, patients in group IV were infused with 4.2% solution of L-arginine (Tivortin® intravenously according to the instructions). The results of application of the drug in the period of development of secondary toxic autoaggression are presented. Results A decrease in the velocity of glomerular filtration (GFR) was established as 66 ± 2.1 ml/min in patients of group II. The use of sorbitol on the first day of the study in patients with EIS (Group III) increased the GFR and caused increased diuresis. This study establishes that the filtration fraction of sodium and cation excretion increase under the influence of sorbilact. Clearance of osmotically active substances (OAS) in patients with EIS is reduced by 30-35% of the control level (SIRS). Infusion of sorbilact in patients with EIS during the development of early toxic autoaggression while undergoing standard treatment was accompanied by a statistically significant increase in plasma osmolarity, and the filtration fraction of OAS (in relation to the second group). The combined use of sorbilact and L-arginine resulted in statistically significant increases in GFR and sodium clearance. Conclusion. Under conditions of medium-severe EIS, sorbilact corrects the integrity of lost osmotic and volumiregulatory functions of the kidneys, but does not restore them to the control level, in particular the degree of oppression is determined respectively at the level of 37% and 22% (p˂0.05). The combination of sorbilact with L-arginine according to the corresponding indicators of clearance after the first session reduces the deficit of compensating volumuminescence (up to 28%, p˂0.05) and osmoregulatory (up to 15%, p˂0.05) renal functions. The obtained results are the basis for the introduction of the sorbilact-L-arginine complex into the program of intensive therapy for the purpose of renoprotection.

The physicochemical parameters of blood serum (osmolality, concentration of several ions, total protein, glucose) were studied in 200 children of different age with various forms of pathology. The variability of each parameter was calculated. A high level of stability of the parameters studies was revealed in healthy children and in children with dysfunction of various systems (disease of the respiratory system, gastrointestinal tract, renal and urinary tract, nervous and endocrine systems). However, estimation of their coefficients of variation showed significant individual deviations of these parameters from the average value of the examined patients. This fact reflects the extent of efficacy of activity of different organs and regulatory systems under pathological processes. Combination of clinical and ontogenetic methods of evolutionary physiology in this study opens new possibilities for understanding the nature of regulation of water-salt balance in humans and points out to the expedience of using these approaches in the practical medicine.

There are several difficulties that impede medical support of space missions. We can assume that the long-term space missions will be accompanied by functional changes in the organism. These changes will be a natural reaction to the factors of space flight. Complex action of factors can lead to the development of both non-specific changes (general adaptation syndrome), and specific changes. We analyzed the physiological changes after long space flights and carried out the correlation of these changes with previously identified proteins in the urine. It is possible to determine the mechanisms of adaptation of the human organism to the conditions of life on Earth after a long stay in Earth orbit.

The aim of this study was to evaluate the parameters of renin-angiotensin-aldosterone system, natriuretic peptides, and markers of endothelial function in the early neonatal period and at the age of 3 months in 83 full-term infants of women with chronic hypertension, there were: 60 newborns from women with chronic hypertension of mild to moderate severity (main group) and 23 newborns from women without hypertension (comparison group). The levels of the renin angiotensin-II, aldosterone, natriuretic peptides, endothelin-1, in cord and peripheral blood were determined by immunoassay, the metabolites of stable oxide nitric--by Griess method. The newborn of women with chronic hypertension showed a significant elevation of renin, angiotensin II and brain natriuretic peptide at birth. A statistically significant increase in concentration of atrial natriuretic peptide (aANP1-28) was determined to the 3-5 days of life. Significantly high levels of renin, angiotensin II, endothelin-1 and decreased levels of endogenous nitrite at the age of 3 months of life was found. The results findings suggest that prenatal activation of the renin-angiotensin-aldosterone system of the fetus, continuing to be in the newborn of women with chronic hypertension during the first three months of life. The same infants have the violation of endothelial function to 3 months of age.

Objective: To improve treatment results of women with massive obstetrical blood loss. Subjects and methods: 96 female patients with average and heavy degree preeclampsia worsened massive blood developing were involved into the investigation. The women were divided into two groups: main (n=55) (basic) - it's patients were treated with complex of offered wiays control (n=41) - it's patients were evaluated retrospectively. During the investigation the parameters of hemostasis system and periphery blood values were performed as dynamic evaluations, acidity-basic state and water-electrolyte balance parameters, medical history were monitored., Results: As a result of the investigation it was found out that these offered actions complex application about reducing massive obstetric blood accelerates restoration of clinic, bio-chemical paramnleters during the early post-operating period, Conclusion: The application of the offered methods has reduced both inltraoperative blood loss in women with preeclamsia and use of blood components and the time spent on the hemostasis system correction for all the women of the base group.

50-years study of water-electrolytes exchange, the condition of water environments of the organism and the hormonal regulation in space flights, and also in postflight period or in its on ground modeling (hypokinesia, bed rest, immersion etc.) has shown the important role of the water-salt homeostasis in adaptation of the human and animal organisms to weightlessness. It has been revealed, that in weightlessness conditions for development of negative balance of a liquid (hypohydration) and the basic electrolytes are created. After the termination of long space flights attributes of development adaptive reactions compensating for extracellular liquid's volume come to light. In order to assess the state of the kidneys and water-electrolyte metabolism in cosmonauts and investigators, functional load tests and especial methods of diagnostic were developed. This is the basis for researches directed on improvement of the scheme of correction hydrogenous the status of an organism of the cosmonauts at the different stages of flight.

Interrelationships between monovalent ion transport, the membrane potential, and intracellular ion and water content of an animal cell are analyzed by computation of the balance of ion fluxes across the plasma membrane when the sodium pump and electroconductive channels operate in parallel with NKCC or Na-Cl (NC) symport. The behavior of the system is described in terms of the integral permeability of Cl- channels, the activities of NKCC or NC symport and two dimensionless parameters characterizing the activity of the Na/K pump relative to that of electroconductive channels. The latter two parameters are shown to largely determine the plasma membrane potential and intracellular concentration of K+ and Na+ whereas NC and NKCC symports and the permeability of Cl- channels regulate cell water balance and only slightly influence the intracellular K+/Na+ ratio and membrane potential.

We compared parameters of water-salt balance in Wistar female rats fed normal chows during more than 2 weeks. Potassium content was 1.4-fold higher in diet I than in diet II, and sodium end water content was 3.3- and 7.5-fold higher in diet II than in diet I. Blood osmolality and concentration of Na+, K+, Mg2+ were equal in rats fed different chow. In water-loaded rats (5 ml of water/100 bw per os) fed different chow, urine flow rate did not differ, but solute-free water excretion was higher by 40.2% in the rats fed diet II vs. diet I. The sort of diet did not affect the renal sodium excretion during oral administration of 5 ml 0.9% NaCl per 100 g bw to rats. After vasopressin injection solute-free water reabsorption was 1.5-fold higher in rats fed diet II. Natriuretic and hydruretic effect of exenatide, glucagon-like peptide 1 mimetic, was weaker in rats fed diet I. The data obtained indicate that organism can effectively maintain blood parameters. The modulation of hormone regulatory effects on water and sodium balance was found to depend on the state of organism under diet consumed continuously.

The paper describes the preoperative care in 260 patients with severe phlegmons of maxillofacial area with participation of anesthesiologist and maxillofacial surgeon, allowing to lower considerably risk of anesthesia, intraoperative and early postoperative complications.

The study proposes a model of plant cell as an integral physical and physical-chemical water-dependent system of maximum structural complexity and complete combination of functions. The framework of this model allows analyzing any cell of simpler organization in any degree of reduction, animal cell included. The model justifies priority in the establishment of internal and functional (turgor) cell pressure of its cytoplasmatic solution high-molecular component along the chain: genome-->protein (quality and quantity)-->functional(turgor) pressure-->cell internal pressure-->water content in cell, gradual decrease of which results in cell death.

Neurosurgical interventions on the skull base including craniofacial block-resections were fulfilled on 36 patients aged from 16 through 73 years. Preoperative management of the patients was made mainly at the intensive therapy room. It included correction of water-electrolyte, protein metabolism, anemia. The key moments in the anesthetic management of such operations were maintenance of an adequate level of neurovegetative blockade, sufficient level of volemia by means of creating hypervolemic hemodilution, prophylactics of air embolism.

The method of long-term continuous low-volume infusion of hydroxyethyl starch (low-flow low volume correction HES) is administrated for correction of fluid balance disorders. The method is aimed to improve the outcomes in preeclamsia patients with multiple organ dysfunction and failure, as the most severe manifestation of system inflammatory response syndrome. After 4 days of the intensive care with application of the developed method in patients with preeclamsia the total body water level is decreased to the normal physiological level, the oedemas are significantly reduced or ceased, the haemodynamics stabilizes, which leads to the reduce of neurologic symptoms. The suggested method of low-flow low volume correction HES, in dose of 15 ml/kg/day is a significant addition to the existing methods of homeostasis and preeclampsia correction.

The paper gives the results of studying the processes of free radical and microsomal oxidation, antioxidant defense, and energy and electrolyte metabolism in the workers of OAO "Salavatnefteorgsintez", one of the leading petrochemical plants in the Republic of Bashkortostan. The metabolic processes have been found to substantially change in the persons contacting a mixture of chlorinated and aromatic hydrocarbons under industrial conditions.

K+, Na+ and Cl- balance and K+ (Rb+) and 36Cl fluxes in during apoptosis of U937 cells caused by 0.2 or 1 microM staurosporine were studied by flame emission and radiotracer techniques. It is found that monovalent ion redistribution accounts for 2/3 of all decrease in the amount of intracellular osmolytes in apoptotic cells while 1/3 is due to the loss of other intracellular osmolytes. Na+ gain in apoptotic cells hampers dehydration is caused by K+ and Cl- loss. It is found that the rate of equilibration of 36Cl, Rb+ (K+) and 22Na+ between cells and the medium exceeds significantly the rate of alteration of cell ion content associated with apoptosis. It is concluded that apoptotic changes should be considered as a drift of the balanced ion distribution. Alteration of the ion balance in apoptosis, caused by 0.2 microM staurosporine, is associated with an increase in the uabain-resistant Rb+ (K+) "channel" influx and insignificant alteration of the uabain-sensitive "pump" influx. Stronger apoptosis, induced by 1 microM staurosporine, is associated with a decrease in the pump fluxes and insignificant changes in the "channel" Rb+ (K+) fluxes. Decreasing of the Cl- level in apoptotic cells by a factor 1.4-1.8 is accompanied with a decrease in the flux, by a factor 1.2-1.6.

The balance of K+, Na+ and Cl- fluxes through cell membrane with the Na+/K+ pump, ion channels and NKCC and NC cotransporters is considered. It is shown that all unidirectional K+, Na+ and Cl- fluxes through cell membrane, permeability coefficients of ion channels and membrane potential can be computed for balanced ion distribution between cell and the medium if K+, Na+ and Cl- concentration in cell water and three fluxes are known: total Cl- flux, total K+ influx and ouabain-inhibitable "pump" component of the K+ influx. Changes in the mortovalent ion balance in lymphoid cells U937 induced to apoptosis by 1 microM staurosporine are analyzed as an example. It is found that the apoptotic shift in ion and water balance in studied cells is caused by a decrease in the pump activity which is accompanied by a decrease in the integral permeability of Na+ channels without significant increase in K+ and Cl- channel permeabilities. Computation shows that only a small part of the total fluxes of K+, Na+ and Cl- accounts for the fluxes via NKCC and NC cotransporters. Therefore, cotransport fluxes can not be studied using inhibitors.

The informative worth of sodium, potassium, calcium, magnesium, glucose, and urea levels in the pericardial liquid is analysed. The values of these parameters in a terminal disease of cerebral origin may testify to the absence of disturbances of energy and protein metabolism or electrolyte disbalance in the myocardium. Disturbances of myocardial electrolyte balance, energy and protein metabolism develop in cardiac, pulmonary, and mixed type terminal conditions. Cardiac arrest is associated with impaired myocardial conduction.

The effect of plasma membrane water permeability on the rate of changes in the volume of principal cells of collecting ducts of the outer substantia medullaris under conditions of hypoosmotic shock has been studied. Changes in cell volume were studied by the fluorescent method. It was shown that the hypotonic shock induced a rapid increase in the cell volume with the characteristic time that depended on plasma membrane water permeability. The decrease in volume occurred much more slowly, and the rate of volume decrease directly correlated with the rate of swelling. The inhibition of potassium transport by barium chloride decreased the rate of volume restoration, without affecting substantially the duration of the swelling phase. The inhibition of mercury-sensitive water channels by mercury caused a significant increase in the time of both cell swelling and volume restoration. It was concluded that the state of water channels largely determines the rate of the regulatory response of epithelial cells of collecting ducts to hypoosmotic shock and affects the exchange of cell osmolites.

The results of analyses of content of water, potassium and sodium ions by Yu. A. Medvedev's method (1988) in muscular tissue from damage region in experiment, are represented. These analyses make possible to determine sequences of stab-and-cut wounds inflicted at short time before death.

Diagnosis of emergences is one of the important objectives of laboratory studies. The present views of critical states and of the modes of their correction make requirements for the quality of conducted studies and that of the apparatuses used in these studies. The present study was undertaken to determine the intra- and interseries reproducibility of the quality of measurement on a Rapidlab 800 gas analyzer (Bayer). The study has indicated that intra- and interseries quality control on the Rapidlab 800 gas analyzer (Bayer) complies with the requirements for the analytical devices used to study acid-base and water-electrolyte balances.

In experiments on non-anesthetized rats with administration into stomach of water (5 ml/100 g body mass) direct correlation has been found between an increase of diuresis and excretion of solute free water (r = 0.98, p < 0.01), while after injection to these animals of 5 x 10(-11) M arginine-vasotocin - between an increase of diuresis and simultaneous rise reabsorption of solute free water (r = 0.8, p < 0.01). The rise of diuresis after the vasotocin injection is due to inhibition of sodium re- absorption, with the solute excretion fraction increasing from 2.6 +/- 0.2 % to 11.9 +/- 1.2, p < 0.001. A similar physiological paradox - an increase of diuresis with the simultaneous increase of reabsorption of solute free water - has been revealed at night hours in children with tendency for nocturnal enuresis (r = 0.64, p < 0.01). Mechanism responsible for this phenomenon consists in a rise of diuresis due to a decrease of sodium ion reabsorption in the ascending Henle loop limb. A problem is discussed of the homeostatic significance of a decrease of sodium reabsorption combined with an increase of solute-free water reabsorption; it is suggested that this phenomenon is based on a redistribution of reabsorption inside the nephron - a decrease of ion and water reabsorption in the initial parts of the nephron distal segment and an increase of solute free water reabsorption with the antidiuretic hormone-stimulated high osmotic permeability of terminal parts of renal tubules. An intraperitoneal injection of V1-anatagonist (OPC-21268) decreased the natriuretic component of response to arginine-vasotocin, while injection of V2-antagonist (OPC-31260) eliminated the antidiuretic component.

Reduction of vinculin occurs in the renal medulla under the long-lasting dehydration. The protein content measured in inner medulla of rats of the WAG line under hydration was 92.1 +/- 6.3 in relative units, but it was only 77.6 +/- 2.3 after a 3-day water deprivation. The vinculin content in the inner medullar layer from mutant rats of Brattleboro line incapable of synthesizing vasopressin is essentially higher: it is 188.9 +/- 8.5 in hydrated conditions and drops to 148.4 +/- 7.3 under a 3-day dehydration. The same high level of vinculin is in outer medulla from rats of Brattleboro line: 222.1 +/- 11.8 in hydrated animals and 174.9 +/- 11 after a 3-day dehydration. No differences were revealed in response of vinculin to alternative osmoregulating stimulation in cortex in both rat lines.

The prospective study explored the hemodynamic effects of colloidal solution replacement therapy and the criteria for its safety in patients with acute lung parenchymatous lesions (ALPL) attended by hypoalbuminemia and coagulopathy. There were 68 observations of the effects of colloidal solutions: 20% albumin solution (n=25), freshly frozen plasma (FFP) (n=20), 6% hydroxyethylated starch (HES) 130/0.4 9:1 (n=23). The colloidal solutions were infused at a constant rate; the infusion was stopped until pulmonary wedge pressure (PWP) was 25% greater than its baseline value. Before and after infusion, the parameters of central hemodynamics and oxygen transport, extravascular lung water index (ELWI), pulmonary vascular permeability index (PVPI), and colloid-osmotic pressure (COP) were measured. The infusion volumes were 3.8 +/- 0.4, 13.7 +/- 1.4, and 13.4 +/- 1.3 ml/kg for 20% albumin solution, 6% HES 130/0.4, and FFP, respectively. The PWP-COP gradient increased in all groups. After FFP infusion, there was an increase in ELWI and lung shunt. After 20% albumin solution, there was a delayed increase in ELWI. There was no rise in ELWI after 6% HES administration. In the 20% albumin solution group, the increased ELWI was recorded in patients who had positive baseline PWP-COP gradients (p < 0.05). A combination of higher PVPI and a positive PWP-COP value causes a greater increase in ELWI after 20% albumin solution infusion than in the normal PVPI group. In patients with ALPL, FFP infusion may lead to an increase in the accumulation of extravascular lung water. A negative preinfusion PWP-COP gradient is a safety criterion for the infusion of 20% albumin solution in patients with ALPL. The increased PVPI in combination with a positive PWP-COP gradient is an aggravating factor.

Aim: To study changes in serum concentrations of interleukines (IL) and tumor necrosis factor alpha (TNFa) in the course of acute moderate and severe shigellosis and salmonellosis; to elicit their correlation with disorders of gas and electrolyte blood composition and acid-base balance (ABB)., Material and Methods: A total of 39 patients with salmonellosis and 32 patients with acute shigellosis admitted to infectious hospital N 2 entered the study. The following parameters were assessed: serum concentrations of IL-1, IL-IO and TNFa; ABB, gas and electrolyte blood composition; leucocytic intoxication index (LII). The tests were made on the disease day 2-3 and 6-7 (in severe salmonellosis on day 10-11)., Results: In moderate salmonellesis and acute shigellosis the level of proinflammatory cytokines (IL-1 and TNFa) diminished while in severe acute intestinal infection their concentration was high reflecting imbalance of immune response. Content of IL-10 depends on etiology and severity of the course of infectious process - the highest IL-10 concentrations were found in patients with severe salmonellosis on the disease day 10-11. A direct correlation was confirmed between IL-1, TNFa and LII in the disease onset in all patients with acute intestinal infections., Conclusion: An important role of IL-1, IL-10 and TNFa in pathogenesis of bacterial intestinal infections is confirmed. A correlation exists between blood gas composition and concentration of cytokines.

A study of water-electrolyte exchange, the condition of water milieu of the organism, and the volume- and electrolyte homeostasis regulation in space flights, and also in postflight period has shown the important role of the water-salt homeostasis in adaptation of the human and animal organism to weightlessness. Obviously, downturn of food consumption, renal excretion and the intestine output seem to be caused by suppression of activity of mechanisms of ion deposition. The most intensive changes of the liquid milieu volumes occur in the first days of weightlessness or in its ground simulation. And, with prolonged duration, the changes of extracellular liquid volume and the volume of plasma do not extend. After termination of long space flights, activation of renin-aldosterone systems occurs as well as a decrease in efficiency of antidiuretic hormone, misbalance of pressor/unpressor prostanoids. In the period of re-adaptation after space flights, development of desensitization of kidneys to endogenous ADH occurs. This is the basis for researches directed to improvement of the existing scheme of correction of the hydrogenous status of the astronaut organism in the closing stage of flight.

The circadian rhythm of urine formation was studied in younger (27 +/- 8 yr) and aged (76 +/- 3 yr) males. In 11 younger healthy examined persons a decrease of diuresis during the night as compared with the day time was due to a rise of solute free water reabsorption. In 31 aged males the change of the urine formation rhythm with increase of the nocturnal diuresis is based on a rise of osmolal clearance combined with an increase of the solute free water reabsorption. It is shown that blockade of autacoid secretion leads to normalization of the diuresis circadian rhythm. In 10 aged men, nocturia was due to a decrease of vasopressin secretion which resulted in a decrease of the solute free water reabsorption and an increase of diuresis. The obtained data are considered as an evidence for the role of renal autacoids, alongside with vasopressin, in regulation of the circadian rhythm of the kidney function.

Peculiarities of drinking behavior and ion deposit capacity have been studied in Wistar and OXYS rats using chronic salt loading. Increased arterial blood pressure in OXYS as compared with Wistar rats has been registered already in the age of 1.5 months. The difference between the two lines persisted up to the age of 18 months. Differences in drinking behavior were also detected: saline consumption in Wistar rats was increased up to 99 ml/day, whereas the OXYS rats revealed a diminished salt appetite. It has been found that tissues of the OXYS rats were characterized by pronounced ion deposit capacity under standard diet conditions as well as after intensive sodium input.

We compared the normalizing effects of various methods of hemocorrection on homeostasis in animals with bile peritonitis. Combined treatment with hypochlorite and UV irradiation produced a potent membrane-stabilizing effect and increased adaptability of erythropoiesis.

In chronic experiments on rabbits and rats, parameters of eye homeostasis (intraocular pressure and ocular hydrodynamics) and lungs (water balance and surface-active properties of surfactant) were studied during electrical stimulation of the locus coeruleus against the background of chronic stress induced in rabbits by repeated electrical stimulation of the ventromedial nucleus and in rats by daily immobilization on a platform. Stimulation of the locus coeruleus eliminated ocular hypertension of hypothalamic origin and stress-induced disturbances in surface activity, blood volume, and water balance in the lungs.

Parameters of water-salt balance in Wistar rats were compared on empty stomach and at standard alimentary regimen (satiated animals). On empty stomach, the blood serum osmolatity amounted to 284 +/- 2, while in satiated rats--to 290 +/- 5 mOsm/kg H2O (p < 0.05); sodium ion concentration on empty stomach lower, whereas no difference in potassium concentration was found. After an intragastric administration of water (5 ml/100 g body mass) to unanaesthetised rats, the blood serum osmolality decreased, while diuresis increased to an equal extent in both groups of rats. For 120 min after the water administration the rats on an empty stomach excreted 92.9% of the administrated fluid, whereas the satiated animals--80.7%. The urine composition differed qualitatively: in fasting rats the increment of diuresis was due to a rise of osmotic free water excretion, whereas in satiated rats--to an increase of excretion of osmotic active substances (including Na and K ions) with water and a simultaneous increase of the osmotic free water reabsorption in the kidney. After the water load and injection of 0.005 nmole/100 g body mass of arginine-vasopressin for 2 hr of the study, diuresis in both groups of animals decreased to an equal extent, but in fasting animals this was due mostly to an increase of the osmotic free water. The data obtained indicate that, under conditions of usual alimentary regimen, the response of kidney to the water load had a character of a volume-regulating response, whereas in fasting rats--of the specific osmorequlating one. A problem is discussed of the concept of norm under usual conditions and at certain clinically accepted restrictions, for instance, on empty stomach.

Forty-seven children, aged 2 to 12, with different pathologies, who underwent the abdominal aortography with the bolus introduction of 60% verografin, i.e. a radiopaque contrast agent (RCA), 2-3 ml/kg for 2-3 sec. The osmolar index of blood went up sharply, diuresis increased 7-fold and the aldosterone content topped 2-fold the original level. Simultaneously, the plasma concentration of ketamine was reducing by 2 times faster than in children who were not administered RCA. An additional introduction of an anesthetic made on minute 3 after RCA injection was suggested to modify the anesthesia management scheme.

Different forms of water content were studied in myocardium and liver of male rats at 2-h hypotension and during the early postreanimation period. The phenomenon of redistribution of free and bound water was found. Comparison of water balance during the postreanimation period in the myocardium and liver revealed that normalization of different forms of water occured in myocardium. In hepatocytes oedema remained over the whole period studied.

Specific electrical conductance (SEC) of the hepatic bile in 94 patients with cholelithiasis and concomitant pancreatitis was increased (p < 0.001) and depended on the degree of cholestasis and the morpho-functional state of the liver. The value of SEC of the intraoperative bile higher than 193 S/m is taken for a criterion of the diagnostics of biliary pancreatitis. The electrical conductance of urine during the first 4 days of observation decreased in patients with acute pancreatitis (p < 0.05), impaired carbohydrate metabolism and concomitant renal dysfunction. The value of SEC of urine lower than 238 S/m corresponds to acute pancreatitis. The investigation of physico-chemical indices of biological liquids in patients with a pathology of the pancreato-biliary system allows to objectively control the course of the disease and to correct the treatment methods.

In experiments on the frog Rana temporaria L. isolated colon, it has been shown that the rate of fluid absorption from the lumen is the highest when the lumen contains a hypotonic solution (22.5 mOsm/kg H2O), whereas the rate is lower in case of mucosal Ringer solution (225 mOsm/kg H2O). 10 nmole arginine-vasotocin rises the hypotonic fluid absorption, while 0.1 mumole prostaglandin E2 reduces the fluid absorption. Water content in the colon wall is the highest at mucosal hypotonic Ringer solution. 0.1 mumole prostaglandin E2 decreases hydration of the colon tissue. Secretion of prostaglandins E1 and E2 into the extracellular fluid of the colon has been established. The data obtained indicate participation of vasotocin and prostaglandin E2 in regulation of fluid absorption in the frog colon.

Rats were subjected to intermittent water restriction from the 1st day of pregnancy until delivery. Water, sodium, and potassium contents in the liver, brain, skeletal muscles, heart, kidneys, and subcutaneous fat tissue were determined in the litter. Water and sodium contents in 30-day old rats of the 1st and 2nd generation were found to be comparable to the control values. Water loading resulted in weaker diuretic responses in the former rats as compared with the control. No differences in the renal response were revealed in adult rats.

Three therapeutic complexes were compared clinically in patients with mild arterial hypertension. Complex 1 consisted of dry air--radon baths, bicycle exercise and exposure of the renal projection area to decimetric electromagnetic field. Its efficacy was 90%, mechanism of the hypotensive action is reduction of enhanced activity of the sympathico-adrenal and renin-angiotensin-aldosterone systems, improvement of water-mineral metabolism and lipid peroxidation. Complex 2 consisted of dry effervescent baths, anaprilin electrophoresis with sinusoidal modulated currents and exposure of the renal projection area to low-frequency alternating magnetic field. Its efficacy was 80%. It affects renin-angiotensin-aldosterone system, water-mineral metabolism and lipid peroxidation. Complex 3 consisted of electric sleep, laser therapy and general sodium chloride baths. Its efficacy was 63%. The effect was due to inhibition of high sympathico-adrenal system.

Based on his experience of treatment of more than 5000 diabetic patients with surgical diseases, the author describes basic principles of management of these patients before, during and after operation. Combined treatment is based on compensation of diabetes, fluid-and-electrolyte balance and acid-base equilibrium. Blood glucose before operation must be limited to 8.3-8.7 mmol/l, ketoacidosis must be absent. During 3-4 days after operation doses of insulin must increase from 0.4-0.5 to 0.8-0.9 U/kg. Energy value of the diet must be not less than 9375-10,450 kJ bearing in mind glucose administered intravenously. Choice of the anesthetic method which has the least negative influence on carbohydrate metabolism and hemostasis as a whole is analyzed. Diabetic patients with surgical diseases can be operated with favourable outcome only in compensated diabetes and balanced homeostasis.

Hydrogen clearance, polarographic, and electric impedance measurements on conscious rabbits with electrodes implanted into brain cortex, thalamus, and hypothalamus were used to study the state of hemodynamics, oxygen and water--electrolyte balance on a model of the zero-gravity-induced cerebrovascular disorders. The preliminary administration of mildronate (10 mg/kg, p.o.) reduced the hemodynamics disorder and optimized the oxygen balance more significantly than did riboxin (5 mg/kg). Both drugs markedly decreased the level of edematous phenomena in the brain. The antihypoxant and antiedematous effects of mildronate and riboxin increased upon their combined administration.

Effect of different concentrations of salts on natural and recombinant strains of Bacillus subtilis and Escherichia coli was studied. The recombinant strain of B. subtilis was found to be more osmotolerant than the wild-type strain of this bacterium, whereas the opposite situation was observed for the recombinant and wild-type strains of E. coli. Some salts exerted a bacteriostatic effect on E. coli and B. subtilis. The adaptive capacity of recombinant strains depended on the number of plasmid copies in the cells. The introduction of recombinant bacteria into model ecosystems resulted in the generation of their variants with increased osmotolerance.

Water deprivation (WD) decreased the serotonin (5-HT) level and significantly increased the 5-hydroxyindoleacetic acid (5-HIAA) in the rat midbrain and hypothalamus, the catabolic 5-HIAA/5-HT ratio increasing three-fold. Hydration (H) produced a moderate increase in the 5-HT and 5-HIAA levels in the hypothalamus with no changes in the 5-HIAA/5-HT ratio. Hydration exerted no significant effect upon the 5-HT level and metabolism in the midbrain. A two-fold increase of corticosterone concentration in water deprivation and its decrease in hydration were shown to occur in peripheral blood plasma.

A significant amount of specific proteins are involved in kidney's response to vasopressin. A relative content of 120 kDa protein in the Wistar rat kidney medulla following a 3-day dehydration, was found to be 0.975 +/- 0.037 and after drinking 4% sucrose solution alone--0.871 +/- 0.038. The difference of protein 120 kDa content was not revealed in Brattleboro rats: in the control rats it was 0.814 +/- 0.044, and in the dehydrated ones--0.854 +/- 0.020. The findings suggest that vasopressin directly regulates the 120 kDa protein level in the kidney inner medulla.

Despite the importance of phagocytosis studying for purposes of physiology and medicine, a physical cause of the initial stage of phagosome generation still remains apparently unclear. The original mathematical theory (Rubinstein, 1998) allows to suggest that the disturbance of osmotic equilibrium, resulting from the interaction between lysosomal hydrolases and their specific substrates, may cause the initial stage of phagosome formation in all biochemically diverse cases of phagocytosis. The author developed this theory without a collaboration with biologists, except for several consultations. The purpose of this paper is to attract the biologists attention to the main ideas underlying this theory, and to learn to what degree these ideas may be reasonable from the viewpoints of biologists.

Blood concentrations of parathyroid hormone, aldosterone, hydrocortisone, Na+, K+, Ca2+, 24-h urine concentration of Ca2+, blood pressure were measured on day 3 and 7 after extracorporeal shock-wave lithotripsy. A total of 54 patients with nephrolithiasis (NL) were examined. In NL patients with hypertension the above lithotripsy led to a fall in pressure by 15-20%, to correction of initial hormonal and electrolytic unbalance. There were marked changes in the levels of parathyroid hormone, total Ca2+ in the blood and 24-h urine.

The method of the blood loss severity estimation, basing on electrochemically registered globular volume deficiency (ECRGVD) was proposed. Classification of an acute blood loss severity accordingly to ECRGVD value was elaborated, general principles of complex treatment of hemorrhage were proposed.

The subjects of the study were both 33 patients treated with a standard basic therapy including the use of diuretics, and 38 patients who at the same time, were treated with intravenous isotonic (5%) solution of glucose of keep blood serum osmolality at the upper normal level. This stabilisation of the blood osmolality has allowed the mortality to be decreased from 21.2 to 7.9% as well as it resulted in a decrease of the frequency of complications.

Comprehensive method for examining the morphology and function of leukocytes is based on the comparison of the size and count of cells before and after incubation of different duration in media with low osmolarity. The method helps assess the leukocyte geometry, the reserve potential of the cytoplasmatic and nuclear membranes, the regulatory features and osmotic stability of cells.
