1. Extraskeletal mesenchymal chondrosarcoma of the abdomen: A rare clinical presentation

Abstract: Extraskeletal mesenchymal chondrosarcoma (EMCS) is a rare malignant soft tissue tumor of chondroprogenitor cell origin. Originally, it was restricted to the bone only but that is no longer the case. Recent literature reports that 20–33% of these tumors occur at the extraskeletal sites. We report one such case, in which the tumor involved the anterior abdominal wall muscles and also had a large intra- abdominal mass that covered a large part of the peritoneal cavity. The clinical features and computed tomography findings suggested the diagnosis of a malignant desmoid tumor with intra-abdominal extension; however, the histopathological examination and the immunohistochemistry proved the tumor to be EMCS. The case is reported due to the dilemma in diagnosis, its rarity, large size, parietal, and intra-abdominal extension with multiple site involvement.

Abstract: Purpose: Scurvy is a disease that has ancient relevance but is a rare occurrence in the present times, due to the advancements in studies and convenient access to professional nutritionists and other services that provide guidance and plans to fulfil all nutritional requirements. In this case report, the authors present the case of a 68 year old male who was diagnosed with scurvy. The diagnosis and treatment of this disease has seen to be greatly delayed due to the infrequency of the cases reported, causing serious consequences for the patient. Scurvy presents with a variety of symptoms affecting the skin and soft tissues such as perifollicular hemorrhages, petechiae, bruising, and arthalgrias. Methodology: The patient was prescribed 250 mg vitamin C supplements (also called Ascorbic acid) 3 times a day and was monitored after 2 weeks to note signs of improvements. Physical examination included observing petechial haemorrhages and bruises. Blood test should be recommended to assess vitamin C levels in blood after the prescribed duration of time. Findings: The signs and symptoms were resolved after regular intake of ascorbic acid supplements for 2 weeks. Recommendation: The findings signifies the need for consideration of scurvy as a possible diagnosis for petechial rash and easy bruising along with other differentials even during the modern era.

Abstract: COG-UK is supported by funding from the MRC part of UK Research & Innovation, the National Institute of Health Research (Grant code MC_PC_19027), and Genome Research Limited, operating as the Welcome Sanger Institute. Background The first epidemic wave of severe acute respiratory syndrome coronavirus-2 (SARS-CoV-2) in Scotland resulted in high case numbers and mortality in care homes. In Lothian, over one-third of care homes reported an outbreak, while there was limited testing of hospital patients discharged to care homes. Aim To investigate patients discharged from hospitals as a source of SARS-CoV-2 introduction into care homes during the first epidemic wave. Methods A clinical review was performed for all patients discharges from hospitals to care homes from 1st March 2020 to 31st May 2020. Episodes were ruled out based on coronavirus disease 2019 (COVID-19) test history, clinical assessment at discharge, whole-genome sequencing (WGS) data and an infectious period of 14 days. Clinical samples were processed for WGS, and consensus genomes generated were used for analysis using Cluster Investigation and Virus Epidemiological Tool software. Patient timelines were obtained using electronic hospital records. Findings In total, 787 patients discharged from hospitals to care homes were identified. Of these, 776 (99%) were ruled out for subsequent introduction of SARS-CoV-2 into care homes. However, for 10 episodes, the results were inconclusive as there was low genomic diversity in consensus genomes or no sequencing data were available. Only one discharge episode had a genomic, time and location link to positive cases during hospital admission, leading to 10 positive cases in their care home. Conclusion The majority of patients discharged from hospitals were ruled out for introduction of SARS-CoV-2 into care homes, highlighting the importance of screening all new admissions when faced with a novel emerging virus and no available vaccine. Abstract: Prevention of respiratory viral infection in stem cell transplant patients is important due to its high risk of adverse outcome. This single-centre, mixed methods study, conducted before the severe acute respiratory syndrome coronavirus-2 pandemic, explored the barriers and facilitators to a policy of universal mask use by visitors and healthcare workers, and examined the impact of the first year of introduction of the policy on respiratory viral infection rates compared with preceding years, adjusted for overall incidence. Education around universal mask use was highlighted as being particularly important in policy implementation. A significant decrease in respiratory viral infection was observed following introduction.

Abstract: Graphical abstract, There is a knowledge gap regarding the factors that impede the ruminal digestion of plant cell walls or if rumen microbiota possess the functional activities to overcome these constraints. Innovative experimental methods were adopted to provide a high-resolution understanding of plant cell wall chemistries, identify higher-order structures that resist microbial digestion, and determine how they interact with the functional activities of the rumen microbiota. We characterized the total tract indigestible residue (TTIR) from cattle fed a low-quality straw diet using two comparative glycomic approaches: ELISA-based glycome profiling and total cell wall glycosidic linkage analysis. We successfully detected numerous and diverse cell wall glycan epitopes in barley straw (BS) and TTIR and determined their relative abundance pre- and post-total tract digestion. Of these, xyloglucans and heteroxylans were of higher abundance in TTIR. To determine if the rumen microbiota can further saccharify the residual plant polysaccharides within TTIR, rumen microbiota from cattle fed a diet containing BS were incubated with BS and TTIR ex vivo in batch cultures. Transcripts coding for carbohydrate-active enzymes (CAZymes) were identified and characterized for their contribution to cell wall digestion based on glycomic analyses, comparative gene expression profiles, and associated CAZyme families. High-resolution phylogenetic fingerprinting of these sequences encoded CAZymes with activities predicted to cleave the primary linkages within heteroxylan and arabinan. This experimental platform provides unprecedented precision in the understanding of forage structure and digestibility, which can be extended to other feed-host systems and inform next-generation solutions to improve the performance of ruminants fed low-quality forages.

Abstract: In this research with the effect of radioactive granite gamma radiation, the reduction of aflatoxin B1 in pistachios was examined in three steps. In the first step, the aflatoxin reduction in small packets by granite bed was tested. In this step, the aflatoxin level of 300 g pistachios packets was reduced up to 81.3 ± 1.5 percent by 4 kg granite bed after 4 days. After observation of aflatoxin reduction by granite bed, the second step was done with increasing the granite and pistachio mass and irradiation time. In this step, the aflatoxin level of 1 kg pistachios was reduced up to 4949 ± 2.6 percent by 6 kg granite after 9 days. According to the results, the aflatoxin reduction of 1 kg pistachios by 1 kg granite after 1 days (as aflatoxin Reduction Coefficient (ARC)) was calculated as ARC = 0.0090 ± 0.0025 (kg. day)−1. The aflatoxin types of detected in this research were B1 and B2 types that AFB2 level was much less than one. Therefore the effect of granite irradiation on AFB2 reduction wasn't considered. The final step was designed for testing the aflatoxin Reduction Coefficient (ARC). This step was shown that the confidence level between practical result and aflatoxin Reduction Coefficient (ARC) result is about 97 percent. The results indicated that the level of fat and protein of pistachios by granite gamma radiation did not change after 9 days. Therefore the granite irradiation can be used for aflatoxin reduction of pistachios.

12. The effect of power plant fuel change on the air pollution (SO2 and NOX ) of surrounding areas: A passive measurement method and health risk assessment

Abstract: Background This study aimed to measure the concentrations of sulfur dioxide (SO2) and nitrogen oxides (NOX) and their relationship with the variables of changing seasons and fuel consumption for Qom Combined Cycle Power Plant in three stations around the power plant from December 2014 to May 2015. Methods In this study, passive sampling was employed in three monitoring sites around the power plant, namely, Pardisan, Mahdiyeh, and Sagheh stations and health risks were assessed for non-carcinogenic effects of exposure to SO2 and NOX via the inhalation route. Results Results showed that the maximum mean concentration values of NOX in winter (when power plants used gasoil) and spring (when power plants used gas fuel) were 52.70 ± 21.70/52.70 ± 13.20 and 22.10 ± 7.00 µg/m3, which were related to Mahdiyeh/Pardisan and Pardisan stations, respectively. In addition, the maximum mean concentration values of SO2 in winter and spring were 5.00 ± 0.20 and 3.90 ± 0.70µg/m3, both for the Mahdiyeh station. Results showed moderately significant correlation between NOx and SO2 concentration values when the power plant consumed gasoil (Spearman rho coefficient 0.68, p 2 Hazard quotients (HQ) values for all age groups were less than 1.00. But the NOx HQ values for less than 2 years old (Infants) and 2–6 years old (Children) were more than 1.00 and for other groups were less than 1.0. Furthermore, the Hazard index (HI) calculation showed that HI for NOx was 3.65, which were > 1, showing an unacceptable chance to human health. Conclusions In conclusion, the use of gasoil by power plants will remarkably increase the emission of pollutants and consequently the concentration of the pollutants in surrounding areas, thereby increasing the health risk for residents of these regions particularly for children which shows the importance of clean fuel supply and the use of emission control technology in the power plant.

Abstract: As the coronavirus disease 2019 (COVID‐19) pandemic continues to evolve, differences in epidemiological and clinical features among pediatrics have been noticed across different countries. We describe the spectrum of COVID‐19 in pediatric patients treated in tertiary health care. We conducted a retrospective chart review of pediatric patients admitted to Indus Hospital & Health care network, Korangi campus, Karachi; from April 1st, 2020 to July 31st, 2020. A total of 141 COVID‐19 cases were reported, males were 81 (57%) and the median age was 8 (0.3–17) years. Moderate and severe infections were noted in 36(26%), and 17(12%) children respectively. Abstract: Introduction: Reducing noise pollution has become an essential issue due to the increase in public concern and also social demands for a better lifestyle. Using sound absorption materials is a preferred method to reduce the noise pollution. Undesirable properties of pure polyurethane such as poor absorption of mechanical energy in narrow frequency ranges can be improved by providing polymeric nanocomposites. The main purpose of this study is to synthesize the polyurethane nanocomposite foams in order to improve its acoustic properties. Material and Methods: At the first steage, pure polyurethane foam was synthesized using the pre-polymer method. Afterwards, nanocomposite foams were synthesized with different mass fractions of Nano silica. The cellular morphology of prepared nanocomposite foams was investigated by scanning electron microscopy (SEM (.Utilizing a two-microphone impedance tube, sound absorption coefficient (α) was calculated in the frequency ranges of 100 Hz to 1600 Hz in order to investigate the acoustic properties of the new absorbant. Results: According to the microscopic investigations, morphology of the cells changed after adding silica nanoparticles. Also, the cell sizes were observed to be decreased by increasing the amount of silica nanoparticles. Furthermore, the acoustic analysis of nanocomposite foams indicated that the sound absorption increased by enhancing the load of silica nanoparticles. Conclusion: In the current study, the effect of silica nanoparticles additive amount on acoustic properties of the polyurethane-based nanocomposite was investigated. Our findings depicted that the polyurethane-based nanocomposites were able to promote the sound absorption coefficient.

Abstract: Summary Background Self-report is one of the most widely used techniques for the assessment of illicit drugs. Nonetheless, the data obtained from illicit drugs user through self-reports is associated with bias and random error. Therefore, validation of these kinds of studies should be determined. Objectives The aim of this study was to determine the agreement between family report of amphetamine/methamphetamine abuse by deceased individual and laboratory findings from autopsy. Methods In this cross-sectional study, demographic, epidemiological and laboratory data of people, whose cause of death was illicit drug use in Iran, between March 2015 and March 2018, were extracted by two checklists. Individuals with both laboratory results and family report regarding the amphetamine/methamphetamine use were extracted and agreement between their results was assessed using the Kappa index, sensitivity, specificity, positive predictive value, negative predictive value, and ROC curve. Results The total number of people who died due to illicit drugs use were 7762. There were 3716 (47.8%) cases with both questionnaire responses and lab results; 88.8% were male and the rest were female. Family reports demonstrated moderately low validity with a low Kappa agreement rate 0.33 (95% CI: 0.30, 0.36), sensitivity 60.3% (95% CI: 56.9–63.7) and moderately high specificity 77.9% (95% CI: 76.4–79.4). Conclusion Results of this study imply that self-report of family cannot be used as a valid response for amphetamine usage in deceases related to illicit drugs use.

Abstract: Corn is one of the main food items for humans and animals. Contamination of corn with aflatoxin during harvest, storage, and transport is one of the human problems. Different methods for removing and inactivating aflatoxin in corn have been introduced so far. In this research, using the gamma radiation caused by radioactive granite, the reduction of corn aflatoxin was investigated with practical and simulation methods. In a practical method by simulation result, the aflatoxin reduction as a function of time and granite gamma radiation dose in corn were calculated. The simulation was done with the Mont Carlo N-Particle X version (MCNPX) code that based on the Monte Carlo method. Results show that the relationship between the percentage of aflatoxin reduction and the irradiation time (t (day)) is 0.017 × t. Due to the low-level gamma dose of granite, the percentage of protein, fat, and vitamins in corn does not change with granite irradiation. Therefore, the results show that the use of low granite gamma radiation to reduce aflatoxin can improve physicochemical properties, reduce aflatoxin levels and increase the antioxidant properties of corn, which has ultimately reduced the risk of developing cancer caused by aflatoxin.

Abstract: Numerous methods have been introduced for digital images watermarking. But there are fewer studies on plenoptic images. This image has more information (such as depth) and costs more than regular digital images. So, protecting the ownership of plenoptic is important. In this paper a robust plenoptic image watermarking has been introduced which is based on Graph-based Transform (GBT). Also, in order to maximizing the robustness of watermarking an optimization has been used to find the best graph structure of GBT. This optimization has done using a Genetic Algorithm. One of the common ways to robust the watermarking is the use of discrete cosine transform (DCT). In this study, we have shown that the proposed method is much more powerful than DCT. The proposed method is tested on three plenoptic images namely Alley, Basic Shapes and Coffee Table and the results show that the proposed method is more robust to similar methods such as DCT methods. Watermarking was evaluated using BER, SSIM and PSNR criteria and Gaussian noise attacks with varying intensities.

Abstract: Background and Objective: Candida species are the second most common cause of vaginitis in worldwide after bacterial agents. This study was performed to determine the etiological factors of Candida vulvovaginitis in pregnant women. Methods: This descriptive-analytical study, was performed on 100 pregnant women with vaginal discomfort that referring to gynecologic and obstetric clinics of the Sanandaj Social Security Hospital, Kurdistan province, Iran during 2016. After obtaining informed consent and collecting background information using a questionnaire, the patients were examined by a specialist physician and samples were collected. Identification of isolates was done based on colony color and also using PCR-restriction fragment length polymorphism (PCR-RFLP) method. Results: Candida vulvovaginitis was seen in 29% of women. Candida albicans were the most common cause of the disease (86.2%) followed by Candida paraposilosis (10.43%) and Candida glabrat (45.3%) . There was no significant relationship between diabetes and candida vulvovaginitis. Abstract: Background/Aims Hepatitis E virus [HEV] is the most common cause of acute hepatitis in developing countries but rare in the industrialised world. In the vast majority it causes an acute self-limiting infection which is often asymptomatic. It is usually detected due to raised transaminases in liver function tests [LFTs] which are checked as part of DMARD monitoring. There is increasing literature that immunocompromised patients with human immunodeficiency virus or transplant recipients are at risk of developing chronic HEV infection, which can progress to liver failure. It is unclear that immunosuppressed patients who have a rheumatological conditions have similar issues, which we have looked into locally. Chronic infection is defined as having detectable RNA for over 3 months. Methods Retrospective review of patients attending rheumatology outpatient clinic for blood monitoring, who had positive Hepatitis E serology between 2010 and 2021. Results We identified nine patients attending the rheumatology clinic in Sheffield, confirmed as having acute infection with HEV as Hep E IgM antibody was positive. They were all on immunosuppression and hence having regular monitoring bloods and were receiving either prednisolone, DMARDS or biologics or in combination. There were five males and four females, with an age range of 38 to 71 years. Eight patients had acutely abnormal LFTs with a peak ALT [normal Conclusion Our case series raises awareness that rheumatologists should consider HEV as a differential in patients with a marked unexpected rise in transaminases. We observed that all our patients cleared HEV infection and there was no evidence of chronicity even with ongoing immunosuppression, which is different from Hepatitis B & C. We recommend that immunosuppressant treatment should be stopped initially following HEV infection and reintroduced once LFTs have normalised and HEV RNA is undetectable. Disclosure C.A. Current literature is unclear on whether the Alpha variant is associated with increased severity. Multivariate analysis using logistic regression assessed risk factors for severity as defined by hypoxia at admission.ResultsThere were 5810 SARS-CoV-2 RNA-positive cases of which 2341 were admitted (838 in wave 1 and 1503 in wave 2). Both waves had a temporally aligned rise in nosocomial cases (96 in wave 1 and 137 in wave 2). The Alpha variant was first identified on 15 November 2020 and increased rapidly to comprise 400/472 (85%) of sequenced isolates from admitted cases in wave 2. A multivariate analysis identified risk factors for severity on admission, such as age (OR 1.02, 95% CI 1.01 to 1.03, for every year older; pConclusionsOur analysis is the first in hospitalised cohorts to show increased severity of disease associated with the Alpha variant. The number of nosocomial cases was similar in both waves despite the introduction of many infection control interventions before wave 2.

Abstract: Introduction: The present study aimed to investigate the change of air pollutants in 2020 in Qom compared to the same period in 2019 in five scenarios. Materials and Methods: The hourly air quality data was obtained from air quality monitoring stations of Qom Environmental Protection Organization (EPO). The meteorological parameters were obtained from Iranian Meteorological Organization website. The data were analyzed using Excel, SPSS, and WRPLOT view. Results: In the first month of the COVID-19 crisis, NO2, SO2, and CO decreased by 26.4, 39, and 0.2 µg/m3 compared to same period in 2019, respectively; however, PM2.5 and O3 increased by 7.1 and 2.3 µg/m3, respectively. In Iranian Nowruz holidays, an increase of 2.9 µg/m3 in O3 mean concentration and a decrease of 8.1, 23.8, 22.8, and 0.2 µg/m3 in mean concentration of PM10, NO2, SO2, and CO were experienced. The prevailing wind direction during the 2020 in each scenario was from the west of Qom city. Conclusion: Gaseous pollutants decreased during the crisis, but particulate pollutants increased slightly compared to the same period in 2019. The lockdown may have had the most impact in decreasing pollutants. A slight increase in wind speed from the west could be a factor in increasing particles. This crisis provided an opportunity to assess the role of policies, such as traffic reduction plans or discarding worn-out cars or urban management to improve air quality.

Abstract: Integrated Development Environments (IDEs) provide tool support to automate many source code editing tasks. Traditionally, IDEs use only the spatial context, i.e., the location where the developer is editing, to generate candidate edit recommendations. However, spatial context alone is often not sufficient to confidently predict the developer's next edit, and thus IDEs generate many suggestions at a location. Therefore, IDEs generally do not actively offer suggestions and instead, the developer is usually required to click on a specific icon or menu and then select from a large list of potential suggestions. As a consequence, developers often miss the opportunity to use the tool support because they are not aware it exists or forget to use it. To better understand common patterns in developer behavior and produce better edit recommendations, we can additionally use the temporal context, i.e., the edits that a developer was recently performing. To enable edit recommendations based on temporal context, we present Overwatch, a novel technique for learning edit sequence patterns from traces of developers' edits performed in an IDE. Our experiments show that Overwatch has 78% precision and that Overwatch not only completed edits when developers missed the opportunity to use the IDE tool support but also predicted new edits that have no tool support in the IDE., Comment: 25 pages, 7 Figures, 4 Algorithms, 3 Tables

Abstract: Background: \ud The SARS-CoV-2 delta (B.1.617.2) variant was first detected in England in March, 2021. It has since rapidly become the predominant lineage, owing to high transmissibility. It is suspected that the delta variant is associated with more severe disease than the previously dominant alpha (B.1.1.7) variant. We aimed to characterise the severity of the delta variant compared with the alpha variant by determining the relative risk of hospital attendance outcomes.\ud \ud Methods: \ud This cohort study was done among all patients with COVID-19 in England between March 29 and May 23, 2021, who were identified as being infected with either the alpha or delta SARS-CoV-2 variant through whole-genome sequencing. Individual-level data on these patients were linked to routine health-care datasets on vaccination, emergency care attendance, hospital admission, and mortality (data from Public Health England's Second Generation Surveillance System and COVID-19-associated deaths dataset; the National Immunisation Management System; and NHS Digital Secondary Uses Services and Emergency Care Data Set). The risk for hospital admission and emergency care attendance were compared between patients with sequencing-confirmed delta and alpha variants for the whole cohort and by vaccination status subgroups. Stratified Cox regression was used to adjust for age, sex, ethnicity, deprivation, recent international travel, area of residence, calendar week, and vaccination status.\ud \ud Findings: \ud Individual-level data on 43 338 COVID-19-positive patients (8682 with the delta variant, 34 656 with the alpha variant; median age 31 years [IQR 17–43]) were included in our analysis. 196 (2·3%) patients with the delta variant versus 764 (2·2%) patients with the alpha variant were admitted to hospital within 14 days after the specimen was taken (adjusted hazard ratio [HR] 2·26 [95% CI 1·32–3·89]). 498 (5·7%) patients with the delta variant versus 1448 (4·2%) patients with the alpha variant were admitted to hospital or attended emergency care within 14 days (adjusted HR 1·45 [1·08–1·95]). Most patients were unvaccinated (32 078 [74·0%] across both groups). The HRs for vaccinated patients with the delta variant versus the alpha variant (adjusted HR for hospital admission 1·94 [95% CI 0·47–8·05] and for hospital admission or emergency care attendance 1·58 [0·69–3·61]) were similar to the HRs for unvaccinated patients (2·32 [1·29–4·16] and 1·43 [1·04–1·97]; p=0·82 for both) but the precision for the vaccinated subgroup was low.\ud \ud Interpretation: \ud This large national study found a higher hospital admission or emergency care attendance risk for patients with COVID-19 infected with the delta variant compared with the alpha variant. Results suggest that outbreaks of the delta variant in unvaccinated populations might lead to a greater burden on health-care services than the alpha variant.\ud \ud Funding: \ud Medical Research Council; UK Research and Innovation; Department of Health and Social Care; and National Institute for Health Research.

Abstract: Severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) infections were rising during early summer 2021 in many countries as a result of the Delta variant. We assessed reverse transcription polymerase chain reaction swab positivity in the Real-time Assessment of Community Transmission–1 (REACT-1) study in England. During June and July 2021, we observed sustained exponential growth with an average doubling time of 25 days, driven by complete replacement of the Alpha variant by Delta and by high prevalence at younger, less-vaccinated ages. Prevalence among unvaccinated people [1.21% (95% credible interval 1.03%, 1.41%)] was three times that among double-vaccinated people [0.40% (95% credible interval 0.34%, 0.48%)]. However, after adjusting for age and other variables, vaccine effectiveness for double-vaccinated people was estimated at between ~50% and ~60% during this period in England. Increased social mixing in the presence of Delta had the potential to generate sustained growth in infections, even at high levels of vaccination.

Abstract: We identify amino acid variants within dominant SARS-CoV-2 T-cell epitopes by interrogating global sequence data. Several variants within nucleocapsid and ORF3a epitopes have arisen independently in multiple lineages and result in loss of recognition by epitope-specific T-cells assessed by IFN-γ and cytotoxic killing assays. Complete loss of T-cell responsiveness was seen due to Q213K in the A*01:01-restricted CD8+ ORF3a epitope FTSDYYQLY207-215, due to P13L, P13S and P13T in the B*27:05-restricted CD8+ nucleocapsid epitope QRNAPRITF9-17, and due to T362I and P365S in the A*03:01/A*11:01-restricted CD8+ nucleocapsid epitope KTFPPTEPK361-369. CD8+ T-cell lines unable to recognise variant epitopes have diverse T-cell receptor repertoires. These data demonstrate the potential for T-cell evasion and highlight the need for ongoing surveillance for variants capable of escaping T-cell as well as humoral immunity., Graphical Abstract

Abstract: B.1.1.7 lineage SARS-CoV-2 is more transmissible, leads to greater clinical severity, and results in modest reductions in antibody neutralization. Subgenomic RNA (sgRNA) is produced by discontinuous transcription of the SARS-CoV-2 genome. Applying our tool (periscope) to ARTIC Network Oxford Nanopore Technologies genomic sequencing data from 4400 SARS-CoV-2 positive clinical samples, we show that normalised sgRNA is significantly increased in B.1.1.7 (alpha) infections (n = 879). This increase is seen over the previous dominant lineage in the UK, B.1.177 (n = 943), which is independent of genomic reads, E cycle threshold and days since symptom onset at sampling. A noncanonical sgRNA which could represent ORF9b is found in 98.4% of B.1.1.7 SARS-CoV-2 infections compared with only 13.8% of other lineages, with a 16-fold increase in median sgRNA abundance. We demonstrate that ORF9b protein levels are increased 6-fold in B.1.1.7 compared to a B lineage virus in vitro. We hypothesise that increased ORF9b in B.1.1.7 is a direct consequence of a triple nucleotide mutation in nucleocapsid (28280:GAT > CAT, D3L) creating a transcription regulatory-like sequence complementary to a region 3’ of the genomic leader. These findings provide a unique insight into the biology of B.1.1.7 and support monitoring of sgRNA profiles to evaluate emerging potential variants of concern.

Abstract: Structured abstractObjectivesTo characterise within-hospital SARS-CoV-2 transmission across two waves of the COVID-19 pandemic.DesignA retrospective Bayesian modelling study to reconstruct transmission chains amongst 2181 patients and healthcare workers using combined viral genomic and epidemiological data.SettingA large UK NHS Trust with over 1400 beds and employing approximately 17,000 staff.Participants780 patients and 522 staff testing SARS-CoV-2 positive between 1st March 2020 and 25th July 2020 (Wave 1); and 580 patients and 299 staff testing SARS-CoV-2 positive between 30th November 2020 and 24th January 2021 (Wave 2).Main outcome measuresTransmission pairs including who-infected-whom; location of transmission events in hospital; number of secondary cases from each individual, including differences in onward transmission from community and hospital onset patient cases.ResultsStaff-to-staff transmission was estimated to be the most frequent transmission type during Wave 1 (31.6% of observed hospital-acquired infections; 95% CI 26.9 to 35.8%), decreasing to 12.9% (95% CI 9.5 to 15.9%) in Wave 2. Patient-to-patient transmissions increased from 27.1% in Wave 1 (95% CI 23.3 to 31.4%) to 52.1% (95% CI 48.0 to 57.1%) in Wave 2, to become the predominant transmission type. Over 50% of hospital-acquired infections were concentrated in 8/120 locations in Wave 1 and 10/93 locations in Wave 2. Approximately 40% to 50% of hospital-onset patient cases resulted in onward transmission compared to less than 4% of definite community-acquired cases.ConclusionsPrevention and control measures that evolved during the COVID-19 pandemic may have had a significant impact on reducing infections between healthcare workers, but were insufficient during the second wave to prevent a high number of patient-to-patient transmissions. As hospital-acquired cases appeared to drive most onward transmissions, more frequent and rapid identification and isolation of these cases will be required to break hospital transmission chains in subsequent pandemic waves.

Abstract: We report severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) spike ΔH69/V70 in multiple independent lineages, often occurring after acquisition of receptor binding motif replacements such as N439K and Y453F, known to increase binding affinity to the ACE2 receptor and confer antibody escape. In vitro, we show that, although ΔH69/V70 itself is not an antibody evasion mechanism, it increases infectivity associated with enhanced incorporation of cleaved spike into virions. ΔH69/V70 is able to partially rescue infectivity of spike proteins that have acquired N439K and Y453F escape mutations by increased spike incorporation. In addition, replacement of the H69 and V70 residues in the Alpha variant B.1.1.7 spike (where ΔH69/V70 occurs naturally) impairs spike incorporation and entry efficiency of the B.1.1.7 spike pseudotyped virus. Alpha variant B.1.1.7 spike mediates faster kinetics of cell-cell fusion than wild-type Wuhan-1 D614G, dependent on ΔH69/V70. Therefore, as ΔH69/V70 compensates for immune escape mutations that impair infectivity, continued surveillance for deletions with functional effects is warranted., Graphical abstract, Meng et al. report that the SARS-CoV-2 spike ΔH69/V70 has arisen multiple times. The deletion increases entry efficiency associated with increased cleaved spike in virions and can compensate for loss of infectivity. The B.1.1.7 spike requires ΔH69/V70 for efficient cell entry and cell-cell fusion activity.

Abstract: LamPORE is a novel diagnostic platform for the detection of severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) RNA combining loop-mediated isothermal amplification with nanopore sequencing, which could potentially be used to analyze thousands of samples per day on a single instrument. We evaluated the performance of LamPORE against reverse transcriptase PCR (RT-PCR) using RNA extracted from spiked respiratory samples and stored nose and throat swabs collected at two UK hospitals. The limit of detection of LamPORE was 10 genome copies/μl of extracted RNA, which is above the limit achievable by RT-PCR, but was not associated with a significant reduction of sensitivity in clinical samples. Positive clinical specimens came mostly from patients with acute symptomatic infection, and among them, LamPORE had a diagnostic sensitivity of 99.1% (226/228; 95% confidence interval [CI], 96.9% to 99.9%). Among negative clinical specimens, including 153 with other respiratory pathogens detected, LamPORE had a diagnostic specificity of 99.6% (278/279; 98.0% to 100.0%). Overall, 1.4% (7/514; 0.5% to 2.9%) of samples produced an indeterminate result on first testing, and repeat LamPORE testing on the same RNA extract had a reproducibility of 96.8% (478/494; 94.8% to 98.1%). LamPORE has a similar performance as RT-PCR for the diagnosis of SARS-CoV-2 infection in symptomatic patients and offers a promising approach to high-throughput testing.

Abstract: Background: The SARS-CoV-2 variant B.1.1.7 was first identified in December, 2020, in England. We aimed to investigate whether increases in the proportion of infections with this variant are associated with differences in symptoms or disease course, reinfection rates, or transmissibility. Methods: We did an ecological study to examine the association between the regional proportion of infections with the SARS-CoV-2 B.1.1.7 variant and reported symptoms, disease course, rates of reinfection, and transmissibility. Data on types and duration of symptoms were obtained from longitudinal reports from users of the COVID Symptom Study app who reported a positive test for COVID-19 between Sept 28 and Dec 27, 2020 (during which the prevalence of B.1.1.7 increased most notably in parts of the UK). From this dataset, we also estimated the frequency of possible reinfection, defined as the presence of two reported positive tests separated by more than 90 days with a period of reporting no symptoms for more than 7 days before the second positive test. The proportion of SARS-CoV-2 infections with the B.1.1.7 variant across the UK was estimated with use of genomic data from the COVID-19 Genomics UK Consortium and data from Public Health England on spike-gene target failure (a non-specific indicator of the B.1.1.7 variant) in community cases in England. We used linear regression to examine the association between reported symptoms and proportion of B.1.1.7. We assessed the Spearman correlation between the proportion of B.1.1.7 cases and number of reinfections over time, and between the number of positive tests and reinfections. We estimated incidence for B.1.1.7 and previous variants, and compared the effective reproduction number, R t, for the two incidence estimates. Findings: From Sept 28 to Dec 27, 2020, positive COVID-19 tests were reported by 36 920 COVID Symptom Study app users whose region was known and who reported as healthy on app sign-up. We found no changes in reported symptoms or disease duration associated with B.1.1.7. For the same period, possible reinfections were identified in 249 (0·7% [95% CI 0·6–0·8]) of 36 509 app users who reported a positive swab test before Oct 1, 2020, but there was no evidence that the frequency of reinfections was higher for the B.1.1.7 variant than for pre-existing variants. Reinfection occurrences were more positively correlated with the overall regional rise in cases (Spearman correlation 0·56–0·69 for South East, London, and East of England) than with the regional increase in the proportion of infections with the B.1.1.7 variant (Spearman correlation 0·38–0·56 in the same regions), suggesting B.1.1.7 does not substantially alter the risk of reinfection. We found a multiplicative increase in the R t of B.1.1.7 by a factor of 1·35 (95% CI 1·02–1·69) relative to pre-existing variants. However, R t fell below 1 during regional and national lockdowns, even in regions with high proportions of infections with the B.1.1.7 variant. Interpretation: The lack of change in symptoms identified in this study indicates that existing testing and surveillance infrastructure do not need to change specifically for the B.1.1.7 variant. In addition, given that there was no apparent increase in the reinfection rate, vaccines are likely to remain effective against the B.1.1.7 variant. Abstract: Background As of January 19, 2021, around two million fatalities and 68 million recoveries from coronavirus disease 2019 (COVID-19) have been reported around the globe. The past pandemics of severe acute respiratory syndrome (SARS) and the Middle East respiratory syndrome (MERS) hint toward a risk of occurrence of "Long-COVID" syndrome, i.e., the persistence of post-discharge symptoms among COVID-19 survivors. With the scarcity of literature addressing post-COVID-19 manifestations and little regard for the stigma associated with this disease, survivors' rehabilitation remains widely neglected. The current study aims to assess the prevalence and characteristics of post-COVID-19 manifestations and their effect on the quality of life (QoL) of COVID-19 recovered individuals. We have also analyzed the relationship of time since the recovery of COVID-19 and its severity with the post-discharge symptoms. The stigma affiliated with the infection of SARS coronavirus-2 (SARS-CoV-2) has also been highlighted. Methodology A descriptive, cross-sectional, questionnaire-based study was conducted from September 2020 to December 2020 among 158 COVID-19 recovered patients, whose information was obtained from Dow Diagnostic Laboratory, Ojha Campus, Karachi, Pakistan. The questionnaire consisted of four sections: sociodemographic data, post-COVID-19 manifestations, questions relating to the stigma, and the QoL of the recovered COVID-19 patients. We used the EuroQol five-dimension five-level questionnaire to assess the QoL, while the modified BG Prasad Socioeconomic Classification updated for 2019 was employed to determine the socioeconomic status of the participants. Data were analyzed using SPSS version 24.0 (IBM Corp., Armonk, NY, USA). Data were presented in the form of frequencies and percentages. Results An overwhelming majority (94.9%) experienced at least one post-COVID-19 symptom, with fatigue (82.9%) being the most prevalent post-discharge manifestation. We observed a significant correlation of post-COVID-19 symptoms with gender, age, and time since recovery. COVID-19 severity was found to be significantly related to the five dimensions of the QoL. A significant difference in EuroQol Visual Analog Scale health score was observed between the participants with mild, moderate, and severe COVID-19 infection (p < 0.001). Besides, the associated stigma with SARS-CoV-2 infection was found to be more prevalent in the participants belonging to the upper class as compared to the other classes (p < 0.05). Nonetheless, we also observed a significant association of disease severity with post-COVID-19 manifestations and pre-existing comorbidities. Conclusions The long-COVID syndrome is similar to the post-discharge manifestations of the survivors of prior pandemics of SARS and MERS. Multi-disciplinary rehabilitation teams, healthcare workers, and the general population should recognize the need for systematic assessment of their recovery and further rehabilitation.

Abstract: SARS-CoV-2 lineage B.1.1.7 viruses are more transmissible, may lead to greater clinical severity, and result in modest reductions in antibody neutralization. subgenomic RNA (sgRNA) is produced by discontinuous transcription of the SARS-CoV-2 genome and is a crucial step in the SARS-CoV-2 life cycle. Applying our tool (periscope) to ARTIC Network Oxford Nanopore genomic sequencing data from 4400 SARS-CoV-2 positive clinical samples, we show that normalised sgRNA expression profiles are significantly increased in B.1.1.7 infections (n=879). This increase is seen over the previous dominant circulating lineage in the UK, B.1.177 (n=943), which is independent of genomic reads, E gene cycle threshold and days since symptom onset at sampling. A noncanonical sgRNA which could represent ORF9b is found in 98.4% of B.1.1.7 SARS-CoV-2 infections compared with only 13.8% of other lineages, with a 16-fold increase in median expression. We hypothesise that this is a direct consequence of a triple nucleotide mutation in nucleocapsid (28280:GAT>CAT, D3L) creating a transcription regulatory-like sequence complementary to a region 3’ of the genomic leader. These findings provide a unique insight into the biology of B.1.1.7 and support monitoring of sgRNA profiles in sequence data to evaluate emerging potential variants of concern.One Sentence SummaryThe recently emerged and more transmissible SARS-CoV-2 lineage B.1.1.7 shows greater subgenomic RNA expression in clinical infections and enhanced expression of a noncanonical subgenomic RNA near ORF9b.

Abstract: This study aimed to assess the awareness and performance of Qom citizens towards using disinfectants and compared its relationship with geographical distribution of COVID-19 outbreak in Qom, Iran. The study was conducted by a researcher-made questionnaire during April and May, 2020. COVID-19 incidence data for each district of city was obtained from health department of Qom province. Data were analyzed using Excel, SPSS and ArcView (GIS) softwares. It was found that the highest level of citizens’ awareness (52%) was in the weak range while their performance (56%) was in the good range. According to Spearman’s correlation analysis, there was a strong correlation (rho 0.95) between the total mean of awareness and performance (p &lt, 0.01). The highest incidence rate of COVID-19 was in district 7 which had the lowest mean score in both awareness and performance. In addition, the results of ANOVA (LSD—least significant difference) showed that there was a significant difference (p &lt, 0.05) between district 7—with lower mean scores in awareness and performance—and other districts. Overall, it is concluded that citizens’ awareness level was lower than that of their performance. This conclusion not only calls for more training programs to be implemented in public places, schools, universities and governmental offices, but it also necessitates maintaining a proper and timely training about using disinfectants.

Abstract: Global dispersal and increasing frequency of the SARS-CoV-2 spike protein variant D614G are suggestive of a selective advantage but may also be due to a random founder effect. We investigate the hypothesis for positive selection of spike D614G in the United Kingdom using more than 25,000 whole genome SARS-CoV-2 sequences. Despite the availability of a large dataset, well represented by both spike 614 variants, not all approaches showed a conclusive signal of positive selection. Population genetic analysis indicates that 614G increases in frequency relative to 614D in a manner consistent with a selective advantage. We do not find any indication that patients infected with the spike 614G variant have higher COVID-19 mortality or clinical severity, but 614G is associated with higher viral load and younger age of patients. Abstract: Amoebiasis is a common infection widely prevalent in tropical countries with low income and poor sanitation. The clinical picture is usually nonspecific; however, invasion of the liver by Entamoeba histolytica could lead to an amoebic liver abscess (ALA). It is relatively uncommon in women and children. Though rare, extension of ALA into the lungs, pleural cavity, and pericardium may prove fatal. Pericardial amoebiasis is a rare complication which, if not treated early, could result in cardiac tamponade and subsequent death. The standard management option is eradication with metronidazole along with the drainage of fluid from the liver abscess and pericardial effusion. Herein, we present a case of a seven-year-old male child with ALA, who developed signs and symptoms suggesting pericardial effusion within a few days of hospital admission. Early diagnosis of pericardial complication and successful management of abscess resolved the pericardial effusion.

Abstract: Hereditary tyrosinemia type 1 (HT-1) is a rare autosomal recessive disorder caused by a deficiency in the enzyme fumarylacetoacetate hydrolase (FAH), which catalyzes the final step in the tyrosine degradation pathway. Hereditary tyrosinemia is a heterogeneous disease with a wide spectrum of clinical manifestations involving hepatic, renal, or nervous systems. It has grave consequences if left untreated. Some of the late complications of hereditary tyrosinemia include cirrhosis, liver nodules, hepatocellular carcinoma, hypophosphatemic rickets, nephrocalcinosis, glomerulosclerosis, and chronic renal failure. Rarely, infants with hereditary tyrosinemia may present with persistent hypoglycemia, which may be a result of acute liver failure or hyperinsulinism. Hyperinsulinemic hypoglycemia (HH), caused by dysregulation of insulin secretion from pancreatic β-cells, leads to insulin driven glucose entry into the tissues and inhibits glycolysis, gluconeogenesis, fatty acid release, and ketone body synthesis. Hyperinsulinemic hypoglycemia can cause severe, persistent hypoketotic hypoglycemia. Diagnosing tyrosinemia type 1 can be a challenge as it is a heterogeneous disorder with a wide variety of clinical manifestations and complications. We herein report a rare case of a three-day-old male neonate with HT-1 compounded with HH.

41. New mechanistic insight into the digestion of complex dietary fibre by rumen microbiota using combinatorial high-resolution glycomic and transcriptomic analyses

Abstract: Background The rumen microbial community is considered the most efficient anaerobic digestive ecosystem known, yet less than half of the energy in low quality forages is actually metabolized. There is a knowledge gap regarding the specific factors that impede the ruminal digestion of plant cell walls or if rumen microbiota have the functional potential and activities to overcome these constraints. To address these issues, innovative experimental methods may provide a high-resolution understanding of the cell wall chemistries and higher-order structures that are resistant to microbial digestion and how they interact with the functional activities of the rumen microbial community. Results With this goal, we characterized the total tract indigestible residue (TTIR) from cattle fed a high-forage diet containing low-quality straw using two comparative glycomic approaches: ELISA-based glycome profiling and glycosidic linkage analysis. Using these techniques, we successfully detected numerous and diverse cell wall glycan epitopes in barley straw and TTIR and determined their relative abundance pre- and post-intestinal digestion. Of these, xyloglucans and heteroxylans were the most recalcitrant to digestion. Linkage analysis identified indigestible linkages consistent with the polysaccharide epitopes identified by ELISA-based glycome analysis. To determine if residual plant polysaccharides within TTIR could be metabolised, rumen microbiota from cannulated cattle fed barley straw were incubated with barley straw and TTIR in in vitro batch cultures. Transcript coding for carbohydrate-active enzymes (CAZymes) were identified and characterized for their contribution to cell wall digestion based on glycomic analyses, comparative gene expression profiles, and associated CAZyme families. High-resolution phylogenetic fingerprinting of these sequences revealed encoded enzymes with activities predicted to cleave the primary linkages within heteroxylan and arabinan. Conclusion This experimental platform provides unprecedented precision in the understanding of forage structure and digestibility, which can inform next-generation solutions to improve the growth of ruminants fed low quality forages and enhance the use of crop residues as a feedstock.

Abstract: Background Malnutrition is a significant public health concern and a leading contributor to the global burden of children’s diseases, affecting 50 to 150 million children under the age of five years worldwide. Globally, undernutrition accounts for approximately 33% of the deaths among under-fives. South Asia alone contributes to 50% and 38.8% of the world’s population of wasted and stunted children, respectively. In Pakistan, malnutrition is the leading cause of childhood mortality, accounting for nearly 35% of all deaths under five years of age. Severe acute malnutrition (SAM), the most severe form of malnutrition, is often associated with electrolyte imbalances. This study aimed to determine the frequency of electrolyte imbalance in children with SAM admitted at a tertiary care hospital. Methods This cross-sectional study includes 184 patients with SAM aged between 6 and 60 months, who were admitted at the inpatient Department of Pediatrics, Dr. Ruth K. M. Pfau, Civil Hospital, Karachi, Pakistan, from January 17, 2017 to July 16, 2017. Weight and length/height were measured, and weight-for-height was calculated. Children were labeled to have SAM when weight-for-height was below -3 standard deviation (SD). Blood samples for serum electrolytes were drawn and sent to the lab. Descriptive statistics were calculated, and stratification was performed using the chi-square test. A p-value of ≤0.05 was considered statistically significant. Results The mean age of participants was 22.63 ± 12.71 months. Of the 184 patients with SAM, 172 (93.5%) patients had electrolyte imbalance. Hypokalemia was present in 79.9%, whereas hypocalcemia, hyponatremia, and hypomagnesemia were present in 71.7%, 48.9%, and 13.6%, respectively. Post-stratification results showed a significant association of electrolyte imbalance with gender (p = 0.005) and educational status of parents (p = 0.001). Conclusions Electrolyte disturbances are common in SAM. Serum electrolytes of every malnourished child admitted should be assessed and corrected to avoid fatal outcomes. We suggest that more research with better study designs should be conducted to develop policies and strategies for successfully combating malnutrition in Pakistan. In the meantime, we recommend adopting national guidelines for the management of acute malnutrition to reduce morbidity and mortality.

Abstract: Summary A SARS-CoV-2 variant carrying the Spike protein amino acid change D614G has become the most prevalent form in the global pandemic. Dynamic tracking of variant frequencies revealed a recurrent pattern of G614 increase at multiple geographic levels: national, regional and municipal. The shift occurred even in local epidemics where the original D614 form was well established prior to the introduction of the G614 variant. The consistency of this pattern was highly statistically significant, suggesting that the G614 variant may have a fitness advantage. We found that the G614 variant grows to higher titer as pseudotyped virions. In infected individuals G614 is associated with lower RT-PCR cycle thresholds, suggestive of higher upper respiratory tract viral loads, although not with increased disease severity. These findings illuminate changes important for a mechanistic understanding of the virus, and support continuing surveillance of Spike mutations to aid in the development of immunological interventions., Highlights: - A SARS-CoV-2 variant with Spike G614 has replaced D614 as the dominant pandemic form - The consistent increase of G614 at regional levels may indicate a fitness advantage - G614 is associated with lower RT PCR Ct’s, suggestive of higher viral loads in patients - The G614 variant grows to higher titers as pseudotyped virions

Abstract: We have developed periscope, a tool for the detection and quantification of sub-genomic RNA (sgRNA) in SARS-CoV-2 genomic sequence data. The translation of the SARS-CoV-2 RNA genome for most open reading frames (ORFs) occurs via RNA intermediates termed “sub-genomic RNAs”. sgRNAs are produced through discontinuous transcription which relies on homology between transcription regulatory sequences (TRS-B) upstream of the ORF start codons and that of the TRS-L which is located in the 5’ UTR. TRS-L is immediately preceded by a leader sequence. This leader sequence is therefore found at the 5’ end of all sgRNA. We applied periscope to 1,155 SARS-CoV-2 genomes from Sheffield, UK and validated our findings using orthogonal datasets and in vitro cell systems. Using a simple local alignment to detect reads which contain the leader sequence we were able to identify and quantify reads arising from canonical and non-canonical sgRNA. We were able to detect all canonical sgRNAs at expected abundances, with the exception of ORF10. A number of recurrent non-canonical sgRNAs are detected. We show that the results are reproducible using technical replicates and determine the optimum number of reads for sgRNA analysis. In VeroE6 ACE2+/− cell lines, periscope can detect the changes in the kinetics of sgRNA in orthogonal sequencing datasets. Finally, variants found in genomic RNA are transmitted to sgRNAs with high fidelity in most cases. This tool can be applied to all sequenced COVID-19 samples worldwide to provide comprehensive analysis of SARS-CoV-2 sgRNA.

Abstract: Surface chemistry is an important factor for quality control during production of nanomaterials and for controlling their behavior in applications and when released into the environment. Here we report a comparison of four methods for quantifying amine functional groups on silica nanoparticles (NPs). Two colorimetric assays are examined, ninhydrin and 4-nitrobenzaldehyde, which are convenient for routine analysis and report on reagent accessible amines. Results from the study of a range of commercial NPs with different sizes and surface loadings show that the assays account for 50–100% of the total amine content, as determined by dissolution of NPs under basic conditions and quantification by solution-state ¹H NMR. To validate the surface quantification by the colorimetric assays, the NPs are modified with a trifluoromethylated benzaldehyde probe to enhance sensitivity for quantitative ¹⁹F solid state NMR and X-ray photoelectron spectroscopy (XPS). Good agreement between the assays and the determination from solid-state NMR is reinforced by elemental ratios from XPS, which indicate that in most cases the difference between total and accessible amine content reflects amines that are outside the depth probed by XPS. Overall the combined results serve to validate the relatively simple colorimetric assays and indicate that the reactions are efficient at quantifying surface amines, by contrast to some other covalent modifications that have been employed for functional group quantification.

Abstract: Healthcare workers (HCW) are potentially at increased risk of infection with coronavirus disease (COVID-19) and may transmit severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) to vulnerable patients. We present results from staff testing at Sheffield Teaching Hospitals NHS Foundation Trust, United Kingdom. Between 16 and 29 March 2020, 1,533 symptomatic HCW were tested, of whom 282 (18%) were positive for SARS-CoV-2. Testing HCW is a crucial strategy to optimise staffing levels during this outbreak.

Abstract: Introduction: Migraine is a prevalent and disabling disease with an incompletely understood etiopathology. Aims and objectives: The basic aim of the study is to analyse the migraine polygenic risk score associates with efficacy of migraine-specific drugs. Material and methods: This descriptive study was conducted in Baqai Medical University, Karachi during August 2019 till December 2019. The data was collected with the permission of ethical committee of hospital. The data was collected through a qualitative method. All patients with migraine were interviewed face to face or by telephone by a trained physician or trained senior medical student using a semi-structured interview. The semistructured interview included questions covering the necessary clinical data for migraine diagnoses and information on the effect of migraine treatment. Results: The data consisting of 200 patients. The male:female ratio in patients with migraine was 1:4.7; this was slightly lower in MO (1:5.8) than in MA (1:3.8) (p= 2.7 × 10−4). The patients with migraine were on average 44.2 years old with an SD of 12.8. There was no significant difference in age (SD) between MO and MA (44.0 [12.1] years and 44.4 [13.6] years, respectively). A higher response rate was found for MO than MA in acute and prophylactic treatment response. Conclusion: It is concluded that PRS may be useful in the investigation of shared genetic risk with comorbidities, in studying the relation between primary headache disorders and their sub-forms, and to personalize migraine treatment.
