Les effets secondaires endocriniens de la radiothérapie sont le plus souvent tardifs et nécessitent un suivi prolongé par l'oncologue-radiothérapeute. Les principales glandes endocrines sont réparties au sein de l'organisme et peuvent être inclues dans les faisceaux d'irradiation de nombreuses tumeurs. La symptomatologie pouvant parfois être d'installation insidieuse et peu spécifique, le dépistage de ces effets secondaires s'appuie sur un examen clinique orienté mais aussi sur des dosages hormonaux systématiques. La thyroïde est un organe particulièrement radiosensible et l'hypothyroïdie est observée généralement au-delà de 30 Gy. Après une irradiation cervicale, il est recommandé de réaliser un dosage de thyroid-stimulating hormone (TSH) tous les 6 à 12 mois à vie. Le risque de cancer thyroïdien secondaire ne concerne que la population pédiatrique et existe pour des doses peu élevées, un dépistage systématique est donc nécessaire. Le risque de d'insuffisance antéhypophysaire dépend de la dose, avec une sensibilité différente pour chaque axe. Chez l'enfant l'enjeu principal est le dépistage précoce d'une insuffisance somatotrope afin de prévenir le risque de petite taille définitive. La fonction reproductrice peut être altérée dès 4–6 Gy, nécessitant une préservation de fertilité. Les effets secondaires endocriniens après radiothérapie peuvent être traités dans le but d'améliorer la qualité de vie des patients, c'est pourquoi nous proposons des conduites à tenir afin d'en favoriser le dépistage et le traitement. Endocrine complications after radiotherapy are usually delayed and require prolonged follow-up by the radiation oncologist. Endocrine glands are dispersed throughout the body and can be included in the radiation field of several tumors. As the symptomatology can sometimes be insidious and non-specific, their screening is based on a directed clinical examination but also on systematic hormonal assays. The thyroid gland is particularly radiosensitive, and hypothyroidism is generally observed for doses of more than 30 Gy. After cervical irradiation, it is recommended to perform a TSH assay every 6 to 12 months. The risk of secondary thyroid cancer only concerns children and exists even at low doses, systematic screening is required. The risk of pituitary insufficiency is dose-dependent, with different sensitivity for each axis. In children, the main concern is the early detection of somatotropic insufficiency in order to prevent the risk of short stature. Reproductive function can be impaired after receiving 4–6 Gy requiring fertility preservation. Endocrine side effects can be treated to improve quality of life; therefore, we propose several approaches to be followed in order to promote screening and treatment. [ABSTRACT FROM AUTHOR]

Purpose: This study aimed to evaluate the pre- and post-operative quality-of-life of patients submitted to the resection of pituitary adenoma via endoscopic transsphenoidal. Methods: This was a prospective study on patients submitted to endoscopic transsphenoidal pituitary surgery with the harvest of a nasoseptal flap who responded to the questionnaires FV-36 and SNOT-22 in the pre-operative and in months 1, 3 and 6 following the surgical procedure. Results: A total of 42 patients submitted to pituitary adenoma resection surgery via endoscopic transsphenoidal with a nasoseptal flap were recruited. In all of the physical and mental domains (SF-36) evaluated, there was an improvement in the long-term evaluation (6 months), compared to the pre-operative, as well as in the site-specific evaluation (SNOT-22). Conclusion: The global and site-specific questionnaires in the 6th post-operative month follow-up presented an important improvement in all the physical and mental domains evaluated, as well as in nasal function in the perception of the patients submitted to pituitary adenoma resection via endoscopic transsphenoidal, demonstrating the safety and efficiency of the procedure. 13. Sarcoïdose hypophysaire mimant un adénome hypophysaire: à propos d'un cas et revue de la littérature

L'atteinte isolée de l'axe hypothalamo-hypophysaire dans la sarcoïdose est rare. Seulement quelques cas sont rapportés sur la littérature. Nous rapportons l'observation clinique d'une patiente âgée de 50 ans, opérée il y a 4 ans pour goitre avec thyroïdectomie totale sous traitement substitutif. Elle se plaint de céphalées chroniques depuis 6 mois devenues récemment rebelles concomitant avec la survenue d'une baisse de l'acuité visuelle. L'examen clinique retrouve un syndrome opto-chiasmatique avec altération importante du champ visuel. Elle présente aussi une paralysie du nerf occulo-moteur commun à droite avec ptôsis. L'IRM cérébrale a objectivé un processus sellaire de signal tissulaire avec prise de contraste hétérogène et qui s'étend à la tige pituitaire avec épaississement de celle-ci. Le bilan hormonal pré opératoire a retrouvé une légère insuffisance thyréotrope. La patiente a bénéficié d'une exérèse tumorale large par voie transnasale trans-sphénoidale endoscopique d'une tumeur de consistance fibreuse et peu hémorragique. L'évolution post opératoire était marquée par la survenue d'un diabète insipide et d'une fistule de LCR au 2ème jour post opératoire. L'évolution était bonne sous traitement médical avec drainage spinal. Le bilan général n'a pas retrouvé une autre localisation de la sarcoïdose notamment une tomodensitométrie thoraco-abdominale et échographie cardiaque sans particularités. Le dosage de l'enzyme de conversion est revenu normal. La patiente a bénéficié d'une corticothérapie pour le traitement de la maladie systémique. La neurosarcoïdose est un critère de mauvais pronostic chez un patient atteint d'une sarcoïdose. L'atteinte hypothalamo-hypophysaire est rare avec des complications plus fréquentes que les autres atteintes neurologiques et systémiques nécessitant une prise en charge multidisciplinaire à long terme.

14. Structure-fonction de l’action pionnière de Pax7

Dans l’hypophyse, Pax7 spécifie l’état du lobe intermédiaire grâce à son action pionnière. Cette action implique dans un premier temps une liaison de cette protéine à certains sites d’hétérochromatine et, par la suite, le remodelage de cette hétérochromatine en euchromatine. Le remodelage de l’épigénome par Pax7 permet au facteur de transcription Tpit d’entrainer l’activation du transcriptome mélanotrope par opposition au destin hypophysaire corticotrope entraîné par Tpit en l’absence de Pax7. Pour étudier l’action pionnière de Pax7, différents membres du laboratoire Drouin ont utilisé un modèle de cellules corticotropes. L’expression ectopique du facteur pionnier Pax7 dans ces cellules permet leur transition d’un phénotype corticotrope à mélanotrope. En plus de l’isoforme prédominante de Pax7 exprimée dans hypophyse avec lequel les travaux de Budry et Mayran ont étaient faits, il existe trois autres isoformes de Pax7 qui diffèrent par quelques acides aminés (Q et GL) dans le domaine PAIRED liant l’ADN. Nous voulions savoir quels étaient les domaines protéiques de Pax7 impliqués pour son activité pionnière. Nous avons d’abord étudié les propriétés des différents isoformes naturelles de Pax7 présentant des variations dans le domaine PAIRED que ce soient pour leurs capacités de facteur transcription ou de facteur pionnier dans un modèle de cellules de corticotrope. Nous avons constaté que les quatre isoformes de Pax7, ainsi que Pax3, le facteur le plus homologue à Pax7 dans la famille Pax, présentent une capacité similaire à lier la chromatine fermée. Cependant, Pax3 et les isoformes de Pax7 (GL+) sont incapables d’ouvrir la chromatine sur un sous-groupe d’enhancers pionniers nécessaire à l’activation du transcriptome mélanotrope. Par la suite, par analyse structure-fonction, nous avons identifié une région de Pax7 importante pour sa liaison à soixante pour cent des sites présents dans l’hétérochromatine qui sont ciblés par l’action pionnière et une région importante pour l’ouverture de chromatine aux sites des pionniers sensibles à la variation GL. Collectivement, nos données ont montré que les acides aminés GL dans le domaine PAIRED en N-terminal et la partie C-terminale de la protéine Pax7 sont critiques pour l’ouverture de la chromatine à des enhancers spécifiques présents dans l’hétérochromatine dont l’activation est nécessaire au destin mélanotrope. D’autre part, les analyses de structure-fonctions nous ont permis d’identifier une région critique de Pax7 nécessaire à son interaction avec l’hétérochromatine sur ces même enhancers., In the pituitary, Pax7 specifies the intermediate lobe state through a pioneer factor action that involves the binding of this protein to certain heterochromatin sites and, subsequently, the remodeling of this heterochromatin into euchromatin. Epigenome remodeling by Pax7 thus allows the transcription factor Tpit to determine the melanotrope fate as opposed to the corticotrope pituitary fate that is driven by Tpit in absence of Pax7. To study the pioneering action of Pax7, Budry et al, and Mayran et al, used a model of corticotrope cells which, following the expression of Pax7, transit from a corticotrope phenotype to melanotic phenotype thanks to its pioneering action. In addition to the predominant Pax7 isoform expressed in pituitary with which the work of Budry and Mayran were done, there are three other Pax7 isoforms that differ by a few amino acids (Q and GL) in the DNA-binding paired domain. We wanted to know what protein domains of Pax7 was involved in its pioneering activity. We have investigated the properties of the different Pax7 isoforms with respect to their transcriptional and pioneer capabilities in a model of corticotrope cells. We found that all four Pax7 isoforms together with the closely related factor Pax3, exhibit a similar ability to bind closed chromatin. However, Pax3 and the isoforms of Pax7 (GL+) failed to open the chromatin at a subset of enhancer necessary to activate the melanotrope transcriptome. In addition, we have identified by structure-function analysis a region of Pax7 that appears important for Pax7 binding at 60 percent of heterochromatin sites that are targeted for pioneering and one region important for chromatin opening to the pioneer sites GL-sensible. Collectively, our data suggest that the amino acids GL in the paired domain and the C-terminal part are critical for the chromatin opening to heterochromatin sites necessary to melanotrope transcriptome. On the other hand, structure-functions analyses allowed us to identify a critical region of Pax7 required for the interaction with heterochromatin on these same enhancers.

15. Interdisziplinäres Management von Hypophysentumoren.

Sellar masses may present with an impairment of pituitary function (hypopituitarism), hormone hypersecretion (prolactinoma, acromegaly, glucocorticoid excess) or neurological symptoms (visual impairment, headache). An increasing number of them is discovered as an incidentaloma. Among the various entities, benign pituitary adenomas and cystic lesions are most frequently encountered. The work-up includes a laboratory evaluation for hormone hyper- or hyposecretion and an MRI of the pituitary gland. If the optic chiasm is compromised, a visual field examination is mandatory. Except for prolactinomas, symptomatic sellar masses are usually resected via an endoscopic transsphenoidal approach. If a total resection is not feasible because of the invasion of surrounding structures, debulking to relieve pressure from the optic chiasm is the primary goal and radiotherapy may be considered. Residual hormone excess can be treated medically. In the early postoperative period special attention to the development and treatment of disordered body water homeostasis and hypopituitarism is crucial. Isolated involvement of the hypothalamic-pituitary axis in patients with sarcoidosis is rare. Only a few cases have been reported in the literature. We report the clinical case of a 50-years old female patient who had undergone total thyroidectomy followed by replacement therapy because of goitre 4 years before. She complained of chronic headaches persisting for 6 months and resistant to all appropriate therapies, with concomitant decrease in visual acuity. Clinical examination showed optochiasmatic syndrome with extensive visual field defect and common oculomotor nerve palsy in the right eye with ptosis. Brain MRI objectified pathological process to the sellar region with heterogeneous tissue signal intensity extending to the pituitary stalk with thickening in the latter. Preoperative hormonal assessment showed mild thyrotropic deficiency. The patient underwent wide transnasal endoscopic transsphenoidal resection of fibrous and little bleeding tumor. Postoperative outcome was marked by the occurrence of diabetes insipidus and CSF fistula two days after surgery. Patient’s outcome was good under medical treatment with spinal drainage. Thoracoabdominal CT scan and cardiac ultrasound were performed which showed no other site of occurrence of sarcoidosis. Dose of converting enzyme was normal. The patient received corticosteroid therapy for the treatment of systemic disease. Neurosarcoidosis is a criterion of poor prognosis in a patient with sarcoidosis. Hypothalamopituitary involvement is rare resulting in complications which are more f requent than those of other neurological and systemic disorders. This study was performed to explain how the molecular processes governing the biosynthesis of gonadotropin-releasing hormone (GnRH) and GnRH receptor (GnRHR) in the hypothalamic-pituitary unit are reflected by luteinizing hormone (LH) secretion in sheep during anoestrous period and during luteal and follicular phases of the oestrous cycle. Using an enzyme-linked immunosorbent assay (ELISA), we analyzed the levels of GnRH and GnRHR in preoptic area (POA), anterior (AH) and ventromedial hypothalamus (VM), stalk-median eminence (SME), and GnRHR in the anterior pituitary gland (AP). Radioimmunoassay has also been used to define changes in plasma LH concentrations. The study provides evidence that the levels of GnRH in the whole hypothalamus of anoestrous ewes were lower than that in sheep during the follicular phase of the oestrous cycle (POA: p < 0.001, AH: p < 0.001, VM: p < 0.01, SME: p < 0.001) and not always than in luteal phase animals (POA: p < 0.05, SME: p < 0.05). It has also been demonstrated that the GnRHR amount in the hypothalamus-anterior pituitary unit, as well as LH level, in the blood in anoestrous ewes were significantly lower than those detected in animals of both cyclic groups. Our data suggest that decrease in LH secretion during the long photoperiod in sheep may be due to low translational activity of genes encoding both GnRH and GnRHR. [ABSTRACT FROM AUTHOR]

BACKGROUND AND OBJECTIVE: As an important achievement in industrialization and urbanization, depression can be much related to the oral hygiene especially periodontal indexes which are a symbol of tooth and oral hygiene and their surrounding tissues. The purpose of this study was to compare the periodontal indexes in individuals with and without depression disorders.METHODS: The present study was carried out in the form of a questionnaire, examination, and recording in the information form. The subjects were divided into two groups of 50 a group included the patients suffering from depression who hospitalized in Neuropsychology ward of Panje Azar hospital in Gorgan, Iran and the control group included either the patients who referred to department of oral medicine and diagnosis of Babol dental school or the people accompanying the patients who had claimed to have no history of depression. The periodontal indexes of the study included: Silness and Loe plaque index, Loe gingival index, pocket depth analysis, as well as bleeding on probing. The information related to each patient was recorded in the form and then two groups were compared.FINDINGS: In the present study, the pocket probing were 2.7692±0.5991 and 2.2917±0.4643 in the control and case groups, respectively (p= 0.009). The Plaque Index means were 1.5385±0.1 and 7763.2593±0.5257 in the control and case groups, respectively. Furthermore, the GI and BOP difference between the two groups were not significant. Moreover, there was no significant relationship between the mean of all the analyzed indexes and the increase of depression.CONCLUSION: The results of the present study indicate a vivid relationship between the depth of pocket probing and depression. International audience; We report a case of malignant melanoma revealed by a metastasis to the pituitary gland. The tumor was misdiagnosed as a pituitary adenoma and aggressive transsphenoidal surgery was complicated by a cerebrospinal fluid fistula. Nine weeks later, the patient presented multiple leptomeningeal and brain metastases spreading from the sellar region. Regarding these observations, we conducted a systematic review of the literature in order to investigate clinicoradiological features that should lead clinicians to suspect pituitary metastasis and how it should impact the surgical management.

International audience; Hypercortisolism usually occurs in women of childbearing age. Even if hypercortisolism theoreticallyleads to infertility, due to altered mechanisms of GnRH secretion, pregnancy can happen.Endocrinologists should be aware of the management of this situation in this context, as it may leadto serious comorbidities for both the mother (gestational diabetes, hypertension...) and the fetus(intra-uterine growth retardation, early birth...). Cushing’s syndrome can also happen duringpregnancy: the comorbidities are the same, but the diagnosis is usually very difficult to perform dueto hormonal changes especially due to hyperestrogenism. This review will detail the mechanismsleading to infertility in women with Cushing’s syndrome, the risks of hypercortisolism duringpregnancy, the methods to diagnose Cushing’s syndrome during pregnancy, and the methods ofcontraception.; Le syndrome de Cushing se définit par un hypercortisolisme endogène ou exogène. L’augmentationde sécrétion du cortisol va avoir un impact négatif sur la sécrétion pulsatile de GnRH.L’hyperandrogénie pouvant être associée va également avoir un impact sur la GnRH, ces 2mécanismes aboutissant à un sur-risque d’infertilité. En cas de grossesse survenant chez unepatiente présentant un syndrome de Cushing, de nombreuses conséquences vont pourvoir êtreobservées chez la mère (avec un sur-risque de diabète gestationnel, hypertension artérielle,éclampsie... en sus des complications propres à l’hypercortisolisme) et chez le fœtus (retard decroissance intra-utérin, prématurité...). Il s’agit cependant d’un évènement rare, de diagnosticdifficile, du fait de nombreuses modifications hormonales induites par la grossesse sur le cortisol.L’objectif de cette revue est de détailler les mécanismes de l’infertilité, les méthodes diagnostiquesde syndrome de Cushing pendant la grossesse, les conséquences materno-fœtales, ainsi que lesmoyens de contraception pouvant être proposés.

29. Determination of the Post-Partum Time when the Pituitary-Ovarian Axis of the Suckler Zebu Cow (Bos indicus) is Sensitive to Synthetic GnRH

In order to determine the time of puerperium when the pituitary- ovarian axis of the suckler zebu cow (Bos indicus) is functional, a single dose of 100 μg synthetic GnRH was injected intramuscularly in 60 suckler zebu cows divided into six groups. The animals were treated 7, 10, 15, 20, 25 and 30 days after parturition. Ten milliliters of milk were sampled from each animal as follows: before GnRH injection (A: 1st sample range), on days 5, 6, 7, 8 (B: 2nd sample range) and 20, 21, 22, 23, 24 (C: 3rd sample range) after GnRH injection. In each group, samples obtained before GnRH injection were used as control. Overall, 600 milk samples were used to determine progesterone concentrations by radioimmunoassay. Among the animals treated with GnRH 15 days after parturition, 60% responded to the treatment with a mean progesterone concentration of 2.67 ± 1.89 nmol/L. This response increased along with the length of the post-partum period. Thirty days after calving, all animals responded well to the treatment (mean progesterone concentration: 4.09 ± 1.09 nmol/L) and presented a significant reduction (p < 0.05) in the interval from calving to conception compared to animals treated on days 7 and 10 after calving. This study shows that, as early as 15 days post-partum, the pituitary-ovarian axis of suckler zebu cows is sensitive to exogenous GnRH and that such treatment may improve reproductive performances.

Zusammenfassung Einleitung Nachdem der laborchemische Nachweis erbracht wurde, dass Akupunktur im Bereich der Schädelakupunktur nach Yamamoto (hier ZS-Punkt auf der Yangseite des Schädels) erhöhtes Prolaktin genauso effizient senken kann wie die medikamentöse Behandlung mit Bromocriptin [ 3 ], einem Dopaminagonisten, lag die Überlegung nahe, dass mit der gleichen Akupunktur auch bei PatientInnen mit Morbus Parkinson ein Therapieerfolg erwartet werden kann. Akupunktur am ZS-Punkt auf der Yinseite des Schädels zeigte Symptome bei ParkinsonpatientInnen wie die einer Dopaminüberdosierung. Methode Es wurde eine Studie mit 60 PatientInnen durchgeführt, die an Morbus Parkinson leiden. Die Studienteilnehmer wurden in eine Verumgruppe und eine Kontrollgruppe unterteilt. Die Akupunktur erfolgte in wöchentlichen Abständen an den ZS-Punkten auf der Yin- und der Yangseite des Schädels auf der zum Maximum der Symptome kontralateralen Seite in wöchentlichen Abständen über drei Monate und wurde mittels standardisierter Fragebögen dokumentiert und ausgewertet. Die vorbestehende Medikation wurde beibehalten. Ergebnisse Die Verbesserung der abgefragten Qualitäten des Befindens bei der Verumgruppe lag in einem signifikanten Bereich. Schlussfolgerung Wenn die Akupunktur bei diesem schweren Krankheitsbild zu einer Verbesserung der vielfältigen Symptome beitragen und das Fortschreiten der Erkrankung verzögert werden kann, ist dies für die PatientInnen auch im Hinblick auf die begrenzten medikamentösen Behandlungsmöglichkeiten eine Perspektive für Lebensqualität und Arbeitsfähigkeit. Weitere Studien werden sich mit Langzeitbeobachtungen und multizentrischen Anwendungen dieser Punkte befassen müssen. Introduction Having provided laboratory-chemical evidence of acupuncture in the field of scalp acupuncture according to Yamamoto (in this case: ZS-point on the Yang-side of the cranium) being capable of decreasing raised prolactin just as efficiently as medication with bromocriptine [ 3 ], a dopamine agonist, it stood to reason that the same acupuncture would yield a therapeutic effect when treating patients suffering from Parkinson. In Parkinson patients, acupuncture of the ZS-point on the cranial Yin-side brought forth symptoms, which equaled those of Dopamine overdose. Method A study was carried out including 60 patients suffering from Parkinson. Participants were assigned to a verum-group and a control. For three months, acupuncture was performed weekly to the ZS-points on the Yin-side of the cranium, while the cranial Yang-side contralateral to the maximum of symptoms was chosen for weekly acupuncture over three months. Documentation and evaluation were performed using standardized questionnaires. Pre-existing medication was maintained. Results The verum-group showed statistically significant improvement of required qualities of condition. Conclusion Should acupuncture be capable to aid in the improvement of the many symptoms of this grave condition and to delay its progress, this would give patients a prospect for quality of life and working capacity, in particular when considering the limited chances of pharmaceutic treatment. Les affections endocriniennes du développement sont des maladies rares que soigne et étudiele pédiatre-endocrinologue. Elles nous renseignent sur de nombreux aspects du développement normalet du fonctionnement du système endocrinien.Ce travail regroupe des études cliniques qui concernent différentes glandes endocrines. Toutd’abord l’hypophyse, dont trois aspects sont abordés. Une nouvelle étiologie d’hypopituitarismecongénital syndromique associant anophtalmie et fente palatine est décrite. Le gène impliqué, RAX, estconservé chez tous les vertébrés et était connu chez l’homme pour donner une anophtalmie isolée.L’étude fait le parallélisme entre les phénotypes humain et murin, qui sont remarquablement similaires.Un autre aspect de la pathologie du développement hypophysaire est illustré par une étude del’hypopituitarisme congénital non syndromique dont le diagnostic est fait tardivement en raison descirconstances de vie défavorables des patients. La neurohypophyse est abordée par l’étude de troisfamilles avec diabète insipide central autosomique dominant associé à un retard de croissance. Lapathophysiologie de celui-ci est discutée à la lumière de données de physiologie murine sur l’anorexiepar déshydratation. Ensuite, le syndrome triple A, pléiotrope car lié à un déséquilibre Redox, est étudiésous l’angle de son atteinte rétinienne qui ouvre une fenêtre sur le caractère neurodégénératif de lamaladie. Sheehan's syndrome (SS) or postpartum pituitary necrosis corresponds to a complete or dissociated hypopituitarism, usually precipitated by a brutal or prolonged hemorrhagic shock during the delivery. This condition has become rare in developed countries while it is probably fairly frequent in our country despite the improvement of obstetric conditions. The case reported concerning a young woman of 29 years admitted with an array of acute adrenal insufficiency associated with myxedema coma, neurological disorders involving a cerebellar syndrome, and peripheral neuropathic syndrome, reveals Sheehan syndrome. The clinical course turns rapidly with specific hormone. Acute and severe clinical forms of SS are rare. Acute adrenal insufficiency is the mode by which most acute revelation is described, while myxedema coma remains exceptional. The central and peripheral neurological manifestations of hypopituitarism are known. Although it is rare that these neurological symptoms indicate thyrotropic insufficiency, the central hypothyroidism should not be disregarded as replacement therapy usually provides a spectacular clinical evolution. Le craniopharyngiome est une tumeur épithéliale bénigne prenant naissance dans le cerveau, plus précisément au niveau de l’hypophyse ou de la tige pituitaire. Cette maladie bien que rare, affecte une personne sur cinquante mille. Elle peut être découverte à n’importe quel âge de la vie puisque cette pathologie silencieuse à ses débuts, passe souvent inaperçue en raison de symptômes peu spécifiques. Il s’agit toutefois d’une maladie grave puisqu’elle peut entrainer à terme des séquelles endocriniennes, visuelles, ou neuro intellectuelles. Le craniopharyngiome nécessite une prise en charge à la fois médicale et thérapeutique. Dans un premier temps, le traitement consiste en l’exérèse la plus complète possible de la tumeur mais la proximité immédiate de l’hypophyse avec des structures essentielles en fait une chirurgie très délicate et non dénuée de risque. Outre les risques inhérents à l’opération, le retrait de la tumeur conduit à un dysfonctionnement hypothalamo-hypophysaire plus ou moins complet.

The diagnosis is made only several years after the obstetrical event, but it is known that some of the women who die in the month following the postpartum present clinical signs of hypophyseal insufficiency. Case reports: We here report three cases of TSH secreting pituitary adenoma that highlight the great diversity of the clinical, hormonal and morphological presentation, and their better prognosis. In the presence of an inappropriate TSH secretion, the main differential diagnosis is the syndrome of thyroid hormone resistance. The role of somatostatin analogue prior to the surgical treatment of TSH secreting pituitary adenoma is also underlined in this report. Conclusion: The increasing frequency and early diagnosis of TSH secreting pituitary adenoma may be explained by ultrasensitive methods now used for TSH measurement and progress in pituitary imaging, mainly with MRI. This changing spectrum in the presentation and the excellent response to somatostatin analogues improved in the prognosis of the disease. [ABSTRACT FROM AUTHOR]
