Although these scales are used internationally in individuals with different types of cognitive impairments, it is not known whether observing facial expressions of pain might differ between regions or between different types of cognitive impairments. In a pilot study, facial responses to standardized experimental pressure pain were assessed among individuals with different types of cognitive impairments (dementia, mild cognitive impairment, Huntington's disease, and intellectual disability) from different countries (Denmark, Germany, Italy, Israel, and Spain) and were analyzed using facial descriptors from the PAIC scale (Pain Assessment in Impaired Cognition). We found high inter-rater reliability between observers from different countries. Moreover, facial responses to pain did not differ between individuals with dementia from different countries (Denmark, Germany, and Spain). However, the type of cognitive impairment had a significant impact; with individuals with intellectual disability (all being from Israel) showing the strongest facial responses. Our pilot data suggest that the country of origin does not strongly affect how pain is facially expressed or how facial responses are being scored. However, the type of cognitive impairment showed a clear effect in our pilot study, with elevated facial responses in individuals with intellectual disability.

Existen escasos trabajos realizados en niños y adolescentes con trastorno bipolar (TBP) en los que se evalue la estructura del sueño. Además, los estudios presentan diferencias metodológicas y reportan resultados heterogéneos. El presente trabajo se estructura alrededor de dos estudios originales y una revisión sistemática. Ambos estudios son los primeros en utilizar un grupo control patológico con el objetivo de controlar el efecto que la comorbilidad asociada al Trastorno bipolar pediátrico pueda ejercer sobre la estructura del sueño. En el primer estudio (Estudio I) se evalúan variables polisomnográficas y psicopatológicas en niños y adolescentes con TBP y se comparan con las obtenidas en los niños y adolescentes con Trastorno por déficit de atención e hiperactividad (TDAH) y en niños y adolescentes sin patología psiquiátrica. Hasta donde alcanza nuestro conocimiento, este es el primer estudio polisomnográfico realizado a niños y adolescentes con TBP en fase interepisódica reclutados ambulatoriamente. Son cuatro los objetivos principales del Estudio I. El primero es determinar si el patrón polisomnográfico de los niños y adolescentes con trastorno bipolar nos permite diferenciarlo del de los niños y adolescentes sanos , el segundo es averiguar si la estructura del sueño en niños y adolescentes con trastorno bipolar nos permite diferenciarlos de otros trastornos prevalentes y de difícil diagnóstico diferencial en la edad pediátrica, el tercero es determinar si existen diferencias polisomnográficas entre los niños y adolescentes sin patología psiquiátrica y los niños y adolescentes con TDAH y, el cuarto y último objetivo es evaluar si existe relación entre la sintomatología afectiva y ansiosa y las variables polisomnográficas. El segundo estudio (Estudio II) evalúa el patrón polisomnográfico del Trastorno de disregulación disruptivo del estado de ánimo (T18EA) comparándolo con el obtenido en los niños y adolescentes con TBP y TDAH. Hasta donde alcanza nuestro conoc, Sleep structure research performed in children and adolescents with bipolar disorder (PBD) is scarce. In addition, the studies present methodological differences and report heterogeneous results. The present work contains two original studies and a systematic review. Both studies are the first to use a pathological control group for the comorbidity effect on the sleep structure. In the first study (Study I), polysomnographic and psychiatric variables in children and adolescents with PBD were compared with those obtained in children and adolescents with Attention Deficit Hyperactivity Disorder (ADHD) and in healthy youths. As far as our knowledge is concerned, this is the first polysomnographic study carried out on outpatient euthymic children and adolescents with PBD. Four are the main objectives of this study. The first one is to determine differences between bipolar and healthy groups. The second one is to determine differences between bipolar and ADHD groups. The third one is to determine differences between ADHD and healthy groups. Finally, the last objective is to evaluate the relationship between psychiatric symptoms and polysomnographic variables. Study II evaluates the polysomnographic pattern in youths with Disruptive Mood Dysregulation Disorder (DMDD) and compares it with that obtained in youths with PBD and ADHD. As far as our knowledge is concerned, this is the first polysomnographic study carried out in children and adolescents with DMDD. PBD and DMDD were related and, the relationship between them remains unclear. El proyecto de la tesis doctoral (mención internacional) es un trabajo trifásico de investigación que elabora críticamente el fenómeno de la somatización en entornos de atención primaria de salud entre inmigrantes y nativos (muestra total N = 3,006; inmigrantes N = 1,503 y nativos N = 1,503) en España. Los inmigrantes provienen de cinco regiones étnicas/geográficas: norteafricana, subsahariana, europea oriental, asiática y latinoamericana. El primer estudio explora la relación entre el diagnóstico psiquiátrico y los tipos/frecuencia de síntomas somáticos en los dos grupos de origen, y analiza si los inmigrantes son más propensos a presentar más síntomas somáticos que los pacientes nativos de atención primaria. El segundo estudio, adjunto a la tesis del doctorado, incluye la validación adicional de la "Barcelona Immigration Stress Scale" (BISS) para determinar los factores de riesgo post-migratorios que pueden condicionar la salud mental de los inmigrantes en el país de acogida. El tercer estudio proporciona un análisis pormenorizado de los factores predictivos de somatización en pacientes inmigrantes de atención primaria de salud y contrapartes nativas. El modelo biomédico explicativo de la somatización se contrasta con el modelo cultural, que incluye un paradigma sociocultural de la experiencia y la manifestación de los síntomas somáticos en la atención primaria. Nuestra investigación destaca significativas implic, The current international PhD dissertation project is a three-fold research work that aims to critically elaborate on the phenomenon of somatization in primary health care settings among immigrants and natives (total sample N=3,006; immigrants N=1,503 and natives N=1,503) in Spain. The immigrants come from five ethnic/geographical regions: North African, Sub-Saharan, Eastern European, Asian and Latin American. The first study explores the relationship between psychiatric diagnosis and types/frequency of somatic symptoms in the two groups of origin, and further examines whether immigrants are more prone to exhibit more somatic symptoms than native-born Spanish primary health care patients. The second study, attached to the PhD dissertation, includes the further validation of the Barcelona Immigration Stress Scale (BISS) with a view to determine the post-migration risk factors that may condition the mental health of immigrants in the country. The third study provides a nuanced analysis of the predictive factors of somatization in immigrant primary health care patients and native counterparts. The biomedical explanatory model of somatization is contrasted against the culture-specific one, which includes a socio-cultural paradigm of the experience and manifestation of somatic symptoms in primary care. The quantitative analysis of our clinical data has allowed qualitative interpretations of the observed differences in the presentation of somatic symptoms between the two groups. We suggest that the application of psychiatric criteria in primary health care may conclude to rigid diagnoses that do not allow health professionals to understand the meaning-making of the somatic symptoms and may lead to under-recognition or mis-diagnosis of a wide range of culture-specific mental conditions. Introducció: Les persones que pateixen malalties neurològiques com l'esclerosi múltiple (EM) i la malaltia de Parkinson (MP), poden constituir un grup de risc per la seguretat en la conducció de vehicles. Resulta difícil discriminar quines proves neuropsicològiques de les aplicades habitualment en el seu seguiment clínic es relacionen amb els tests de conducció estàndard aplicats en el nostre país. Amb aquesta finalitat vàrem portar a terme dos estudis. Estudi 1: Es van incloure 50 pacients amb diagnòstic de EM i 50 controls amb similar edat, sexe y nivell educatiu. Els pacients amb diagnòstic d'EM es varen dividir en dos grups en funció de la presencia de deteriorament cognitiu o l'absència del mateix. L'avaluació neuropsicologica es va realitzar mitjançant una extensa bateria normalitzada per a població espanyola. Per l'avaluació de la capacitat de conducció es va aplicar la prova ASDE i l'Useful Field of View (UFOV). Es va valorar la disfunció motora mitjançant l'Expanded Disability Status Scale (EDSS). Resultats: Les proves de conducció varen mostrar una baixa sensibilitat i una alta especificitat per a la detecció dels pacients a risc. Els pacients amb diagnòstic d'EM sense deteriorament cognitiu només varen mostrar diferencies respecte al grup control en les proves de conducció de velocitat de processament visual. En canvi, la presencia de deteriorament cognitiu associat a l'EM va mostrar diferencies en la majoria dels subtests de les proves de conducció. Les proves neuropsicològiques que varen permetre identificar als pacients amb EM i deteriorament cognitiu amb major risc per a la conducció van ser les que impliquen les funcions executives i la velocitat de processament de la informació (TMTA, dígits inversos del WAIS, número de errors del PASAT, clau de nombres del WAIS). El rendiment en aquestes proves juntament amb la disfunció motora va mostrar valor predictiu per al risc en la conducció entre el 47 y 48%. Estudio 2: Se incluyeron 37 pacientes con diagnóstico de EP y 33 sujetos, Introduction Persons suffering from neurological disorders such as multiple sclerosis (ME) and Parkinson's disease (PD) may constitute a risk group for the safety driving of vehicles. It is difficult to discriminate which neuropsychological tests from those usually used in the clinical follow-up of these patients are related to the standard driving tests applied in our country. To this purpose two studies were conducted. Study 1: Fifty patients diagnosed with MS and 50 healthy controls matched by age, sex, and educational level were included. Patients with MS were divided into two groups according to the presence or absence of cognitive impairment. Neuropsychological evaluation was carried out using an extensive battery of tests standardized for the Spanish population. Driving performance tests included the standard Spanish driving test (ASDE) and the Useful Field of View (UFOV). The Expanded Disability Status Scale (EDSS) was used to assess motor dysfunction. Driving tests showed a low sensitivity and a high specificity for the detection of patients at risk. Patients diagnosed with MS without cognitive impairment only showed differences as compared with controls in the visual processing speed. By contrast, patients with cognitive impairment in association with MS showed differences versus healthy controls in most subtests of the driving tests. Neuropsychological tests that allowed identification of patients with MS and cognitive impairment with greater driving risk were those assessing executive functions and information processing speed (TMTA, Digits Forwards of WAIS, number of errors in the PASAT test, and WAIS letter-number sequencing). The performance of these tests together with motor dysfunction showed a predictive value for unsafe driving between 47 and 48%. El trastorno depresivo mayor (TDM) presenta un curso muy heterogéneo y que conlleva un importante sufrimiento y pérdida de funcionalidad a aquellos que lo sufren. El objetivo principal de esta tesis es la identificación de estructuras cerebrales que puedan estar alteradas en primeros episodios depresivos (PED) para su caracterización como marcadores del trastorno y de aquellas estructuras que predecirán y/o mostrarán alteraciones con la aparición de recurrencias. Para ello se ha estudiado la habénula mediante técnicas de neuroimagen estructural, desde un enfoque transversal con el objetivo de encontrar diferencias entre los pacientes en diferentes estadios de la depresión y los controles sanos. Posteriormente se ha realizado un estudio longitudinal mediante morfometría basada en vóxel (VBM) de los pacientes PED para identificar qué estructuras se alteran de forma temprana en la depresión y si se relacionan con la aparición de recurrencias. El primer artículo cuenta con una muestra de 95 pacientes en diferentes estadios de la depresión i.e., PED, pacientes recurrentes en estado de remisión y pacientes con depresión crónica refractaria. Los resultados muestran que la sustancia blanca (SB) de la habénula es significativamente mayor en mujeres con un primer episodio depresivo en comparación con las que sufren depresión crónica y las pacientes control. Entre las mujeres se observa que mayores volúmenes de SB habenular correlacionan con una mayor edad de debut y una menor duración de la enfermedad, aunque este resultado no pudo confirmarse en el análisis por grupos. El segundo artículo cuenta con una muestra de 33 PED y 33 controles sanos a los que se ha seguido durante dos años. Los PED presentan a nivel basal menores volúmenes de SB en el giro frontal superior derecho y la corona radiada anterior que los controles sanos, aunque estas diferencias se desvanecen a lo largo del seguimiento. Los PED que presentan recurrencias (n=15), tienen volúmenes mayores de SB en áreas q, Major depressive disorder (MDD) is characterized by a very heterogeneous course and by a significant suffering and loss of functionality for people who develop it. The main objective of this thesis is to identify those brain structures that could be altered in first depressive episodes (FED) for their characterization as markers of the disorder and those structures that will predict or will show alterations while recurrences appear. To do this, the habenula was studied from a structural transversal neuroimaging approach to find potential differences between patients at different stages of depression and healthy controls. Subsequently, a longitudinal study of FED patients using voxel-based morphometry (VBM) has been carried out to identify which structures could be are altered in early stages of MDD and to identify if they are related to the appearance of recurrences. The first article included 95 patients in different stages of depression i.e., FED, remitted-recurrent and chronic refractory depression patients and healthy controls (HC). The results showed that habenula white matter volume (WMV) was significantly higher in women-FED compared to those who suffered from chronic depression and with healthy control patients. Among the women, it is observed that greater habenular WMV correlate with elderly onset and shorter duration of the disease, although this result could not be confirmed in the analysis by groups. The second article has a sample of 33 FED and 33 HC that have been followed for two years. FED patients present lower WMV in the superior right frontal gyrus and in the anterior corona radiata than the healthy controls at baseline, although these differences seem to vanish during the follow-up. Longitudinal analysis showed that FED patients who suffered recurrences (n = 15) have enlarged WMV in left posterior corona radiata and right posterior thalamic radiation at the end of the follow-up. La teràpia dialèctica conductual (TDC) és una teràpia efectiva per al tractament del trastorn límit de la personalitat (TLP). Algunes de les limitacions a l'hora d'implementar-la són l'elevat nombre de pacients que abandona la teràpia abans de finalitzar-la, i el fet de ser una teràpia llarga i costosa. S'ha observat que el mòdul de mindfulness de la TDC per si sol és eficaç per a disminuir la severitat del TLP, tanmateix no hi ha estudis que avaluïn sobre quin símptoma específic del TLP és eficaç cada un dels mòduls, ni quins possibles correlats neurobiològics rauen als efectes de la teràpia. La present tesis es constitueix de dos blocs. El primer bloc (estudi 1), pretén avaluar els factors relacionats amb l'abandonament prematur de la TDC. El segon bloc (estudis 2 i 3) pretén comparar dos dels mòduls de la TDC: el mòdul de mindfulness (TDC-M) i el mòdul d'efectivitat interpersonal (TDC-EI) entre sí. L'estudi 2 analitza l'eficàcia de cada un dels mòduls sobre els dos símptomes nuclears del TLP, la desregulació emocional i la impulsivitat. L'estudi 3 pretén trobar el correlat neurobiològic associat als beneficis de la teràpia, posant especial atenció als efectes del mindfulness en la default mode network (DMN). Els resultats mostren que la comorbiditat amb trastorn per consum de cocaïna, la comorbiditat amb trastorn de conducta alimentària i les altes puntuacions en impulsivitat prediuen l'abandonament prematur de la psicoteràpia. També mostren que els mòduls de mindfulness i d'efectivitat interpersonal per si sols i per separat són beneficiosos per a disminuir la severitat global del TLP, la desregulació emocional, l'ansietat i la simptomatologia depressiva. En canvi, els beneficis en impulsivitat només es donen en el grup de mindfulness. Respecte els correlats neurobiològics, no observem canvis en la DMN, en canvi sí que observem un increment en l'activitat de la ínsula anterior després de realitzar la teràpia independentment del mòdul. En conjunt els resultats d', Dialectical Behaviour Therapy (DBT) is an effective treatment for people with borderline personality disorder (BPD). Limitations of the therapy include the high number of patients who prematurely drop out from therapy, and its length and associated costs, which gives importance to the study of its components separately. The mindfulness module of DBT by itself has proven effective to reduce BPD severity, however there are no studies that examine which specific symptoms benefit from mindfulness, nor what neurobiological mechanisms may underlie its benefits. The present thesis is constituted by two blocks: The first one (study 1) assesses the factors associated with early dropout from DBT. The second block (studies 2 and 3) compare two of the DBT modules: the mindfulness module (DBT-M) and the interpersonal effectiveness module (DBT-IE). Study 2 assesses the efficacy of every single module in two of the nuclear BPD symptoms, emotional dysregulation and impulsivity. Study 3 assesses the neurobiological correlations associated with the theapy with special attention to the mindfulness effects on the default mode network (DMN). The results show comorbidity with cocaine use disorder, comorbidity with eating disorder and high connections to impulsivity are associated with early dropout from therapy. Both modules DBT-M and DBT-IE separately are effective in decreasing BPD severity, emotional dysregulation, anxiety and depression, however only the mindfulness module was effective in decreasing impulsivity. Regarding neurobiological correlation, changes in the DMN were not observed, however, an increase in activity in the anterior insula was observed after the therapy in both modules. Together the results of the present thesis contribute to knowledge of the factors associated with dropout, and affirm that the modules of mindfulness and interpersonal effectiveness separately are effective to improve BPD symptomatology and that the improvements are associated with changes in brai

No hemos encontrado en la literatura psiquiátrica examinada, estudios de casuística psiquiátrica ambulatoria en los que se haya evaluado su complejidad y gravedad con índices de carga asistencial, así como tampoco ningún estudio donde se clasifique esta casuística ambulatoria basado en variables predictivas de utilización de servicios. La mayor parte de la literatura examinada está relacionada con estudios de altas hospitalarias, aplicando las clasificaciones basadas en los GRD. La gravedad en la clínica psiquiátrica es un concepto poco estudiado y también una realidad clínica difícil de definir, delimitar, describir y calibrar. Hasta ahora, la forma más habitual de definirla se ha basado en la intensidad y la cualidad de los síntomas, así como en su impacto psicosocial (discapacidad). La gravedad de los trastornos mentales se ha estudiado principalmente en relación con trastornos con peor pronóstico, como la esquizofrenia, y en estudios de seguimiento desde unidades hospitalarias, pero no tanto en estudios ambulatorios y en una perspectiva general y comparativa de toda la gama de pacientes psiquiátricos ambulatorios. El Case-Mix o perfil del casos, es un constructo de uso creciente en el campo de la gestión de la salud y una nueva forma de clasificar a los pacientes en la medicina general, de acuerdo con su gravedad, las pautas de gestión clínica, el tratamiento y el consumo de servicios. Hemos abordado en el presente trabajo, y desde la perspectiva del Case-Mix, un sistema de clasificación de pacientes psiquiátricos ambulatorios, basado en escalas de gravedad psiquiátrica y de disfunción psicosocial., We have not found in the psychiatric literature examined, studies of outpatient psychiatric casuistry in which the complexity and severity of it have been evaluated with rates of care burden. Most of the literature examined is related to studies of hospital discharges, applying the classifications based on the GRDs. Gravity in the Psychiatric Clinic is a concept little studied and also a clinical reality difficult to define, delimit, describe and calibrate. Until now, the most usual way to define it has been based on the intensity and quality of the symptoms, as well as their psychosocial impact (disability). The severity of mental disorders has been studied mainly in relation to disorders with worse prognosis such as schizophrenia, and in follow-up studies from hospital units, but not so much in outpatient studies and in a general and comparative perspective of all the range of outpatient psychiatric pathology. The Case Mix or Case Profile, is a construct of increasing use in the field of health management and is a new way of classifying patients in the general medical field, according to their severity, clinical management guidelines and treatment or consumption of services. We have addressed in the present work, and from the perspective of the Case-Mix, a classification system of ambulatory psychiatric patients, based on scales of psychiatric severity and psychosocial dysfunction.

El término patología dual describe la coexistencia de una o más enfermedades mentales y trastornos por el uso de sustancias, la cual es muy común y, a menudo, se asocia con una mayor gravedad psiquiátrica y peores condiciones médicas y sociales. Últimamente ha adquirido relevancia la investigación relacionada con el papel del sistema endocannabinoide (eCB) en las bases neurobiológicas de diversas afecciones neuropsiquiátricas, tales como la ansiedad, los trastornos del estado de ánimo o la esquizofrenia, así como los trastornos relacionados con el uso de sustancias. El propósito de este estudio es investigar la asociación entre varios polimorfismos de los genes que codifican algunas proteínas del sistema endocannabinoide (receptor de cannabinoides 1 [CNR1] y amida hidrolasa de ácido graso [F19H]) de acuerdo con (1) la presencia o ausencia de uso de sustancias trastornos en toda la muestra y (2) según la presencia o ausencia de patología dual en el grupo de sujetos diagnosticados con un trastorno por uso de sustancias. Es un estudio de casos y controles con un tamaño muestral de 675 sujetos, de los cuales 362 son controles (C; individuos sin diagnóstico psiquiátrico) y 313 casos (Cs; individuos con un trastorno por uso de sustancias sin ningún trastorno neuropsiquiátrico comórbido [ TCS] así como individuos con un trastorno de uso de sustancias con una enfermedad neuropsiquiátrica comórbida [PD]). The purpose of this study is to investigate the association between various polymorphisms of the genes encoding for some endocannabinoid system proteins (cannabinoid receptor 1 [CNR1] and fatty acid amide hydrolase [FAAH]) according to (1) the presence or absence of substance use disorders in the whole sample and (2) according to the presence or absence of dual pathology within the group of subjects diagnosed with a substance use disorder. This is a case-control study with a total sample size of 675 subjects, of whom 362 are controls (C; individuals without any psychiatric diagnosis) and 313 cases (Cs; individuals with a substance use disorder without any co-morbid neuropsychiatric condition [TCS] and those individuals with a substance use disorder with co-morbid neuropsychiatric condition [PD]). Molts trastorns del moviment observats en pacients esquizofrènics passen desapercebuts, especialment els de lleu o moderada intensitat o bé els associats amb anosognosia. El valor dels moviments extrapiramidals i específicament de la discinèsia i el parkinsonisme com a factor pronòstic en l'esquizofrènia no és ben conegut. En aquest estudi prospectiu, s'estudia la relació de la discinèsia i altres símptomes extrapiramidals amb l'evolució a un any del trastorn en una cohort de 112 pacients en el seu primer episodi de l'espectre esquizofrènic tractat amb antipsicòtics de segona generació. Els resultats van mostrar que en el model ajustat de regressió lineal, la resposta primerenca al tractament (p = 0.001), l'abús d'alcohol (p = 0.026), el nivell ocupacional més alt aconseguit (p = 0.031), l'edat d'inici més primerenca (p = 0.033), els moviments discinéticos durant les primeres vint i cuatre setmanes (p = 0.034) i el sexe femení (p = 0.039), però no les primeres vuit setmanes de Parkinsonismo, com a variables significatives (R² ajustat 0.416). The role of extrapyramidal movements and specifically dyskinesia and Parkinsonism as prognostic factor in schizophrenia is not well known. In this prospective work the relationship of dyskinesia and other extrapyramidal symptoms with one-year psychiatric outcome was studied in a cohort of 112 patients suffering from First Episode of Schizophrenia Spectrum disorder and treated with Second Generation Antipsychotics. Results showed that, linear regression model of outcome fitted early response (p= 0.001), abuse of alcohol (p= 0.026), higher occupational level reached (p= 0.031), younger age at onset (p= 0.033), dyskinetic movements during the first twenty-four week (p= 0.034) and female sex (p= 0.039), but not first eight weeks Parkinsonism, as significant variables (adjusted R² 0,416). Direction of the association of dyskinesia with severity of illness was negative. Thirty-two patients showed dyskinesia in the first twenty-four weeks and six patients were diagnosed as Tardive Dyskinesia during the follow-up of the study (5.3% out of 112 patients in the initial sample, 8.3% out of the 72 patients that complete the follow-up. The potential role of early dyskinetic movements as a window of the dopaminergic system in schizophrenia is postulated.

Durante las últimas décadas, se está produciendo un importante envejecimiento poblacional que requiere de una atención especializada. Entre la psicopatología que se experimenta entre los adultos mayores, el Trastorno por consumo de sustancias constituye un fenómeno creciente. Sin embargo, sigue infravalorado actualmente, mal identificado e infradiagnosticado y, por todo ello, tratado de forma deficiente. Se ha hipotetizado acerca de la existencia de múltiples factores relacionados tanto biológicos como psicosociales, postulándose acerca de las diferencias atribuibles al sexo del paciente. Así mismo, existe evidencia de que los ancianos con problemática de consumo tienden a presentar consecuencias negativas relacionadas de índole médico y social. Entre estos efectos negativos, destaca la afectación cognitiva relacionada con el consumo crónico de alcohol y fármacos de prescripción y la posibilidad de mejoría tras lograr la abstinencia a sustancias. Finalmente, los estudios indican que tanto la adherencia a los regímenes de tratamiento como la evolución de la patologia y el porcentaje de logro de abstinencia son más favorable entre los pacientes ancianos con problemas de consumo que en la población general. El objetivo de esta tesis doctoral, que se presenta por compendio de publicaciones, es profundizar en el conocimiento del Trastorno por consumo de sustancias en población adulta mayor de 65 años. En los dos primeros artículos centrados en cognición, el consumo de sustancias en mayores demuestra generar consecuencias perjudiciales, alterando diversos dominios cognitivos con el consumo crónico de alcohol y benzodiacepinas. Among the different psychopathologies affecting this group of old adults, substance use disorder keeps increasing. However it is still undervalued, misidentified and infradiagnosed, and therefore does not receive the right treatment. There are several hypotheses around the existence of multiple factors, both biological and psychosocial, related to substance use with sex differences between men and women. There is evidence that old age patients with substance use disorder tend to suffer from negative medical and social consequences. Among these negative effects we can highlight the cognitive impairment related to alcohol and prescription drug abuse, and the recovery after achieving abstinence. Studies indicate that adherence to treatment, progress of the pathology and the percentage of success in achieving abstinence are more favorable amount elderly people compared to general population. The objective of this doctoral thesis, presented as a compendium of publications, is to deepen on the knowledge of substance among in elderly population over 65 years old. In the first two articles focused on cognition, substance use disorder among the elderly demonstrates its harmful consequences, altering several cognitive domains with alcohol and benzodiazepines chronic consumption. Likewise, this secondary cognitive impairment improves when abstinence is achieved during the treatment process. In the third article the characteristics in elderly consumption population are studied, as well as the sociodemographic, clinical and therapeutical differences associated to the sex of the patient that interfere with the type of treatment that should be used. Los Signos Neurológicos Menores (SNM), entendidos como alteraciones neurológicas que no tienen una localización específica a nivel cerebral, presentan una mayor prevalencia en los pacientes con esquizofrenia que en la población sana. También se ha descrito una mayor frecuencia de los mismos en los familiares de los pacientes con esquizofrenia (aunque en menor medida que en los pacientes). Algunos aspectos de la asociación de los SNM con el cuadro clínico de la esquizofrenia permanecen sin esclarecer. Otros temas de interés en la literatura se dirigen hacia la relación entre los SNM y los déficits neuropsicológicos observados en la esquizofrenia y su asociación con los cambios estructurales que caracterizan el trastorno. El objetivo general de esta tesis fue el de estudiar la relación entre los SNM y diversos aspectos de la esquizofrenia en una muestra relativamente amplia de pacientes. El primer estudio examinó los correlatos con: a) los síntomas de la enfermedad organizados en los síntomas positivos (delirios y alucinaciones), el síndrome de desorganización (principalmente trastorno formal del pensamiento) y los síntomas negativos, y b) con el deterioro cognitivo, específicamente con dos aspectos concretos del déficit neuropsicológico observados en la esquizofrenia como son la función ejecutiva y la memoria. El segundo estudio evaluó la relación de los SNM con las estructuras cerebrales, específicamente con los cambios en la sustancia gris. Los SNM fueron valorados en ambos estudios a través de una escala detallada y estructurada, la NES de Buchanan y Heinrichs, que ofrece una puntuación total y tres subescalas: integración sensorial, coordinación motora, secuenciación de actos motores complejos. En el primer estudio se examinó la prevalencia de los SNM en una muestra de 78 sujetos con esquizofrenia crónica. También se exploró un grupo de 36 sujetos controles sanos para su comparación. Los síntomas clínicos fueron evaluados utilizando la escala de síntomas positivo, Neurological soft signs (NSS), defined as minor neurological abnormalities that do not have localizing value, are established as being present at a higher frequency in patients with schizophrenia than in the healthy population. They are also present at a higher than normal frequency in the relatives of patients with schizophrenia, although at a lower rate than in the patients themselves, leading to considerable interest in their potential role as a marker of vulnerability or predisposition to the disorder. Nevertheless, some aspects of the association of NSS with the clinical features of schizophrenia remain unclear. Other relevant issues concern the relationship between NSS and the cognitive impairment and brain structural changes that also characterize the disorder. The general aim of this thesis was to examine the relationship between NSS and selected aspects of schizophrenia in a relatively large sample of patients. The first study examined a) their association with the symptoms of the disorder, as classified into positive symptoms (delusions and hallucinations), the disorganization syndrome (mainly formal thought disorder) and negative symptoms; and b) with the cognitive impairment seen in the disorder, specifically two major specific deficits, executive function and memory. The second study evaluated the relationship of NSS with brain structural abnormality, specifically changes in grey matter. NSS were assessed in both studies using a detailed scale, the Neurological Evaluation Scale (NES) of Buchanan and co-workers, which provides a total score and three subscale scores: sensory integration, motor coordination, and sequencing of complex motor acts. In the first study, the frequency of NSS was rated in a sample of 78 subjects with chronic schizophrenia. A comparison sample of 36 healthy control subjects was also employed. Introducción: La impulsividad es un constructo multidimensional de gran importancia, puesto que se encuentra presente en muchos trastornos psiquiátricos, entre otros, los trastornos por uso de sustancias (TUS), los trastornos antisocial y límite de la personalidad, el comportamiento suicida, los trastornos por control de impulsos o el trastorno por déficit de atención con hiperactividad. Objetivo: El objetivo principal del presente trabajo es explorar y evaluar el papel de la impulsividad en pacientes con trastorno por uso de sustancias que acuden a recibir tratamiento en un centro ambulatorio de adicciones. Metodología: Se seleccionó una muestra de pacientes en tratamiento en la Sección de Adicciones y Patología Dual del Hospital Universitario Vall d'Hebron. Se recogieron variables sociodemográficas, clínicas y otras relacionadas con el consumo. La impulsividad fue evaluada con la Escala de Impulsividad de Barratt (BIS-11), la comorbilidad con la SCID-I y SCID-II y con el C19DID, y la gravedad de la adicción, con el EuropASI. Se diseñó el estudio en diferentes fases. Las tres primeras fases consistieron en un estudio descriptivo, ecológico, transversal, retrospectivo y observacional y, en la segunda fase, se añadió un estudio analítico. En la cuarta fase, se empleó un diseño longitudinal, de cohortes y prospectivo. Resultados: Las puntuaciones de impulsividad en pacientes con trastorno por uso de sustancias son elevadas. La impulsividad se relaciona con la gravedad de la adicción en pacientes dependientes de cocaína y está presente en los pacientes con comorbilidades psiquiátricas y con ideación o intentos de suicidio a lo largo de la vida. La escala BIS-11 tiene un buen funcionamiento a nivel general, pero no ha sido posible poder adaptarla a nuestra población. Conclusiones: La impulsividad en pacientes con trastorno por uso de sustancias es frecuente, y está relacionada con comorbilidades psiquiátricas, comportamiento suicida y con la gravedad de la adicción. Objective: The main objective of this work is to explore and to evaluate the role of impulsivity in patients with SUD in treatment at s an outpatient addiction center (belonging to the Spanish Public Health System) Methodology: A sample of outpatient in treatment at the Addictions and Dual Pathology Section of the Vall d'Hebron University Hospital was selected. Sociodemographic, clinical and other variables related to drug use were collected. Impulsivity was evaluated with the Barratt Impulsivity Scale (BIS-11), comorbidity with the SCID-I, the SCID-II, and the CAADID while addiction severity was assessed with the EuropASI. The study was designed in different phases. The first three phases could be describes as an ecological, cross-sectional, retrospective and observational study and, in the second period, an analytical study was added. In the4th phase, a longitudinal, cohort and prospective study was conducted. Results: Impulsivity scores in patients with SUDs are high. Impulsivity is related to the severity of addiction in cocaine-dependent patients and is present in patients with psychiatric comorbidities as well as among those with ideation or lifetime suicide attempts. The BIS-11 scale has a good performance at a general level, but it has not been possible to be completely adapted to our population. Conclusions: Impulsivity in patients with SUDs is frequent, and is related to psychiatric comorbidities, suicidal behavior and addiction severity. The BIS-11 scale may not be the optimal instrument to assess the impulsivity among patients with addictive disorders. La discinesia tardía se observó por primera vez como un efecto secundario tardío de los fármacos antipsicóticos a finales de la década de 1950. Aparece como movimientos involuntarios simples o complejos, repetitivos e involuntarios que típicamente afectan la cara y las extremidades, aunque puede afectar cualquier grupo muscular del cuerpo. La etiología de la discinesia tardía permanece desconocida, pero dado que a veces se pueden observar movimientos similares en pacientes con esquizofrenia no tratados, actualmente se considera que representa una interacción entre el tratamiento farmacológico y algún aspecto del proceso de la propia enfermedad. The aetiology of tardive dyskinesia remains unknown but based on evidence that similar movements can sometimes be seen in untreated patients with schizophrenia, it is currently considered to represent an interaction between drug treatment and some aspect of the disease process of the disorder itself. Given that the involuntary movements of tardive dyskinesia are broadly similar to those seen in neurological disorders such as choreo-athetosis and dystonia, the basal ganglia have long been suspected to play a key role in its pathogenesis. However, structural imaging studies, first using computerized tomography (CT) and later using magnetic resonance imaging (MRI), have not found consistent evidence that tardive dyskinesia is associated with volume changes in these or other brain structures. The aim of the present study was to re-examine the question of brain structural changes associated tardive dyskinesia, using whole brain, voxel based structural imaging. The design was to compare relatively large groups of patients with schizophrenia with and without tardive dyskinesia. A group of healthy controls were also examined. Thirty-two schizophrenic patients with tardive dyskinesia and 49 patients without tardive dyskinesia, matched on demographic variables, were examined using and whole-brain, optimised voxel-based morphometry. No obstante, el psiquiatra francés Louis Corman (1901-1995) en sus observaciones sobre la estructura craneofacial, afirmaba que una mayor inclinación de la frente hacia atrás se relacionaba con rasgos impulsivos, observación que no ha recibido atención hasta la fecha. Por otro lado, las técnicas de neuroimagen están ayudando a clarificar que tipo de variaciones se producen en las regiones cerebrales implicadas en la impulsividad. Y aunque la investigación se ha centrado mayoritariamente en población clínica, los estudios con población sana parecen estar recibiendo mayor atención en la última década. Así, está investigación se constituyó sobre tres objetivos y cuatro hipótesis. En el primer objetivo se investigó la asociación entre la impulsividad y el grado de inclinación de la frente hacia atrás (GIF), bajo la primera hipótesis que a mayor GIF mayor impulsividad. El segundo objetivo estudió la asociación entre la impulsividad y el volumen de la materia gris (VMG) de toda la corteza cerebral. Se formuló una segunda hipótesis que a menor VMG en regiones fronto-temporales mayor impulsividad. El tercer objetivo investigó la asociación entre GIF y VMG de toda la corteza cerebral y se planteó la tercera hipótesis que a mayor GIF menor VMG en regiones fronto-temporales. Anatomical studies reveal cerebellar connections to motor and non-motor extracerebellar territories; cerebellar injury or degeneration results in a cerebellar motor syndrome but also in a cerebellar cognitive/affective syndrome; neuroimaging studies reveal cerebellar activation in motor and nonmotor processes; and many studies have reported cerebellar abnormalities in numerous neurological and psychiatric diseases that degrade cognition and affect. The compendium of studies that are included in this thesis present the following advances in our understanding of the role of the cerebellum in movement, thought, and emotion: (i) evidence supporting the presence of executive, linguistic, visual spatial, and affective impairments that characterize the Cerebellar Cognitive Affective syndrome, (ii) the description of similarities between linguistic and motor deficits in cerebellar dysfunction, which reinforces the notion of that a uniform computation underlies the cerebellar modulation of movement, thought, and emotion (Universal Cerebellar Transform theory), (iii) a review of the supporting evidence and relevance of the Universal Cerebellar Transform theory, (iv) confirmatory evidence of the presence and arrangement of functionally specialized motor and nonmotor regions of the cerebellum, which reaffirms the anatomical principles that underlie the notion of a Universal Cerebellar Transform, (v) the novel description of triple representation of non-motor task processing in the cerebellum, unmasking new clinical and basic science questions, and (vi) the discovery of a fundamental movement-to-thought principle of cerebellar macro-scale organization, which can serve as a guiding framework for future cerebellar research across neuroscience disciplines.

Introducció: La població llatinoamericana esta permanentment creixent als Estats Units i a Espanya, fet que planteja importants reptes als sistemes de salut pública en ambdós països, des de la limitada accessibilitat als serveis de salut mental i substancies fins a la baixa qualitat i la manca de continuïtat del tractament. La recerca sobre les barreres especifiques de la població llatinoamericana a l'accés i a la retenció als servies de salut mental i de substàncies i el seu rol a la hora de predir l'adherència al tractament es escassa. En aquest projecte de recerca, ens proposem identificar barreres d'accés i retenció i els factors clínics, socials i culturals relacionats a les barreres i als serveis. A més, avaluem com aquests factors i barreres en l'atenció afecten la retenció a un tractament integrador culturalment adaptat per a Llatins amb trastorns de salut mental i substancies. Mètodes: aquest treball de doctorat inclou investigació clínica, amb un estudi d'observació i un d'intervenció i una revisió crítica del tema. Els participants han estat reclutats prospectiva ment a la comunitat en tres ciutats: Boston, Madrid i Barcelona. Després d'examinar els símptomes de salut mental i abús de substàncies, es van recollir dades sobre barreres percebudes, símptomes clínics, coneixements en salut, discriminació i variables sociodemogràfiques. There is a paucity of research that identifies the specific barriers and their role on predicting adherence to mental health and substance abuse treatment within Latino populations and investigates whether these variables change depending on the host country. In this research project, we aim to identify barriers to access and retention and the clinical, social and cultural factors related to them. Moreover, we assess how these factors and barriers in care influence treatment retention of a culturally adapted integrative therapy for Latinos with behavioral disorders. Methods: This research work included a clinical research body, with an observational and an interventional component and a critical review on the topic. Participants were recruited prospectively in the community in three sites; Boston, Madrid and Barcelona. After being screened for mental health and substance abuse symptoms, data on perceived barriers, clinical symptoms, health literacy, discrimination and socio-demographic variables was collected. Eligible participants were randomized for receiving the Integrative Intervention for Dual Problems and Early Action program (IIDEA), a cultural sensitive psychotherapy intervention based on Cognitive Behavioral Therapy (CBT), psychoeducation and mindfulness. Adherence to this intervention was assessed. The observational study that described barriers to retention to treatment among Latinos was first published. A second study assessing the relationship between previously reported barriers to care and adherence to the IIDEA intervention was written and submitted for publication. Finally, a critical review of the literature assessing access and retention to care among Latino immigrants complemented the research project. Results: Barrier

Esta tesis se centra en el estudio de los procesos psicológicos activos en el efecto beneficioso de la meditación y que pueden ser también relevantes en otras prácticas terapéuticas. El foco principal es la capacidad de decentering, descrita como la habilidad para observar los pensamientos y sentimientos propios como sucesos temporales y objetivos en la mente y no como reflexiones sobre el self necesariamente ciertas. La capacidad de decentering es uno de los procesos que ha sido considerado un factor clave para el funcionamiento psicológico saludable y un factor común en diferentes patologías y psicoterapias. El primer trabajo de esta tesis consiste en la validación de la versión en español de la escala Experiences Questionnaire‐Decentering. La escala mostró buenas propiedades psicométricas y también permitió observar que los participantes con diagnóstico psiquiátrico mostraron menores puntuaciones que la muestra no clínica y los sujetos sin experiencia meditativa mostraron menores puntuaciones que los sujetos con experiencia meditativa. Seguidamente plantea la relación entre la práctica meditativa y la conducta valorada y los procesos mediadores en ésta relación. Se confirmó que los sujetos meditadores a diario eran más conscientes de sus valores, consistentes con éstos y se consideraban con mayor plenitud vital. Las medidas sobre valores correlacionaron con los procesos de mindfulness y decentering y pudimos observar su papel mediador entre la práctica meditativa y los valores. El decentering medió en la relación entre práctica meditativa y conducta relacionada con valores. La asociación entre práctica meditativa y vida valorada y plenitud vital resultó mediada tanto por la capacidad de decentering como por las dimensiones de descripción y no juicio del mindfulness. The thesis's main focus is the ability of decentering, described as the ability to observe one's thoughts and feelings as temporary, objective events in the mind, as opposed to reflections of the self that are necessarily true. The ability of decentering is a key process for healthy psychological functioning, hence it is a common factor identified in different pathologies and psychotherapies. The first piece of work presented in this thesis consists on the validation of the Spanish version of the Experiences Questionnaire-Decentering scale. The scale showed good psychometric properties. It also demonstrated that the participants with psychiatric diagnosis scored lower than the non-clinical control group, and that the subjects without meditative experience scored lower than the subjects with meditative experience. We also present here the analysis of the relationship between meditative practice and valued behavior and the mediating processes involved in it. We demonstrated that daily meditators are more aware of their values, more consistent with them and have overall more life fulfillment. The measures of values correlated with the processes of mindfulness and decentering, and we could observe their mediating role amongst meditative practice and values. Decentering mediated the relationship between meditative practice and value-related behavior. The association between meditative practice and valued living and life fulfillment was mediated by both the capacity of decentering and the dimensions of description and nonjudgment of mindfulness. Finally, since ayahuasca's consumption has been demonstrated to have therapeutic potential for depressive symptomatology, substance abuse and complicated mourning; this thesis explores whether the psychological processes underlying the benefits of taking ayahuasca

Els fàrmacs antidepressius necessiten unes quantes setmanes per aconseguir una resposta clínica (Trivedi et al., 2006). Una possible explicació és que l'augment de la concentració de serotonina (5-HT) extracel·lular produïda pels inhibidors de la recaptació de serotonina (ISRS) estigui limitada per l'activació dels autoreceptors 5-HT1A. La utilització de molècules que antagonitzin el receptor 5-HT1A i el dessensibilitzin podria escurçar la resposta antidepressiva i/o augmentar-ne l'efecte. Es realitzen dos assajos clínics utilitzant dues molècules amb acció sobre el 5-HT1A: el Pindolol, un fàrmac amb acció antagonista parcial del receptor 5-HT1A i el DU125530 un nou fàrmac amb acció antagonista completa del receptor 5-HT1A. Es revisa la rellevància del receptor 5-HT1A en el tractament antidepressiu que s'exposa al llarg de la introducció. També es realitza un estudi de metaanàlisi dels assajos clínics controlats realitzats amb pindolol afegit al tractament antidepressiu amb ISRS per l'escurçament i potenciació de la resposta antidepressiva en pacients amb depressió no resistent., Antidepressant drugs require several weeks to achieve a clinically meaningful improvement of depression. One explanation is that the extracellular serotonin (5-HT) concentration produced by reuptake blockade of selective serotonin reuptake inhibitors (SSRI) is limited by the activation of autoreceptors (5-HT1A). Molecules with an antagonism action on 5-HT1A may be useful to enhance or accelerate antidepressive response of SSRI. Two clinical trial are conducted: one with pindolol, a partial 5-HT1A antagonist and one with DU125530 a new full 5-HT1A antagonist. A review about the relevance of 5-HT1A receptor in the SSRI response is also done exposed too in the introduction part of the thesis. A meta-analysis of randomized controlled trials of pindolol augmentation in patients with nonresistant depression is also performed.

Els models de l'aprenentatge de la por tenen un important rol en la comprensió dels trastorns d'ansietat i el seu tractament. Un dels paradigmes més emprats per la recerca de la por és el condicionament clàssic, en el que un estímul inicialment neutre, després de ser associat repetidament amb un estímul incondicionat, es converteix en un estímul condicionat que genera una resposta de por (adquisició de la por). Si l'estímul condicionat es presenta repetidament sense l'estímul incondicionat, la resposta de por disminueix (extinció dins la sessió). Quan el participant torna al context on es va donar l'extinció dins la sessió, apareix de nou la memòria de l'extinció de la por (record de l'extinció). L'extinció de la por (extinció dins la sessió i record de l'extinció) tradicionalment s'ha considerat un procés equivalent a la teràpia d'exposició (el tractament més eficaç per la majoria de trastorns d'ansietat) però aquesta relació pràcticament no ha estat estudiada. Per altra banda, un concepte que ha guanyat reconeixement en relació als trastorns d'ansietat és el control atencional (l'habilitat de regular la distribució atencional, incloent les habilitats per a mantenir l'atenció sostinguda, ignorar distractors i desplaçar el focus atencional entre diferents tasques). En aquest estudi hem explorat, per una banda, si les diferències individuals en l'extinció dins la sessió i el record de l'extinció estaven associades amb els resultats de la teràpia d'exposició. Per a, Fear learning models have a key role in our understanding of anxiety disorders and their treatment. One of the more established paradigms in the research on fear is classical conditioning, in which an initially neutral stimulus, after being repeatedly associated with an unconditioned stimulus, becomes a conditioned stimulus and elicits a fear response (fear acquisition). If the conditioned stimulus is presented repeatedly without the unconditioned stimulus, the fear response shows a decrease (extinction learning). If the subject comes back to the context where extinction learning took place, usually shows a recovery of the fear extinction memory (extinction recall). Fear extinction (extinction learning and recall) has been considered a form of exposure therapy (the most effective treatment for most anxiety disorders) but this is an assumption that has not been tested experimentally. Another concept that has gained recognition among anxiety is attentional control (the ability to regulate attention allocation, including our ability to maintain sustained attention, ignore distractors, and shift attention between tasks). Here, we tested whether individual differences in fear extinction (learning and recall) in the laboratory were associated with the outcomes of an exposure therapy. Furthermore, we studied whether individual differences in fear extinction (learning and recall) were associated with individual differences in non-emotional attentional control. 50 participants with fear of spiders participated in a two-day fear-learning paradigm, and then underwent a brief exposure therapy and performed an attentional control task. Results showed that enhanced extinction learning was associated with better exposure therapy outcome. Moreover, some components of attentional control predicted enhanced extinction learning and recall. La ansiedad se considera actualmente la enfermedad psiquiátrica más prevalente en todos los rangos de edad, y además causa una invalidez considerable, lo que representa una carga para la sociedad. La nosología de los trastornos de ansiedad ha cambiado significativamente durante los últimos siglos. Aunque conceptos antiguos como la "neurosis" abarcaban modelos mixtos en los que los síntomas de ansiedad se interpretaban en el contexto de enfermedades médicas, la nueva clasificación nosológica ha ido migrando hacia los aspectos psicológicos, cognitivos y conductuales de la ansiedad. Sin embargo, la mayor proporción del coste de la ansiedad se atribuye a la pérdida o reducción de la productividad y a los costes directos del tratamiento médico debido a los síntomas físico o somáticos inexplicables. En los últimos años han surgido nuevos hallazgos y nuevos modelos de comorbilidad somática en los trastornos de ansiedad y en esta tesis nos centramos en la relación entre el síndrome de hiperlaxitud articular (también conocido como Ehlers-Danlos tipo Hypermóvil (JHS/hEDS)) y los trastornos de ansiedad, que fue descrito por primera vez hace más de 30 años. The nosology of anxiety disorders has changed significantly during the past centuries. While old concepts like neurosis embraced organic models of anxiety, in which symptoms of anxiety were interpreted in the context of medical illnesses, new nosological classification has moved towards the psychological, cognitive and behavioral aspects of anxiety. However, some studies reported the greatest proportion of the cost of anxiety is attributed to lost or reduced productivity, and the direct costs of medical treatment due to the unexplained physical or somatic symptoms. New models of somatic comorbidity among anxiety disorders have emerged over the past years and in this project, we focus on the relationship between the Joint Hypermobility Syndrome (also known as Ehlers Danlos-Hypermobile type (JHS/hEDS)) and Anxiety disorders, which was described for the first time over 30 years ago. We aimed to further study this association along with the underlying mechanisms with the ultimate goal of understanding the different dimensions of the psychopathology associated with JHS/hEDS. A literature reviews confirm an increasing amount of evidence pointing towards a high prevalence of psychiatric conditions among individuals with JHS/hEDS. Particularly, JHS/hEDS is strongly associated with anxiety and there is also limited but growing evidence that JHS/hEDS is also associated with depression, eating and neuro-developmental disorders as well as alcohol and tobacco misuse. Introducción: Es conocida la elevada psicopatología asociada a los diferentes subtipos de pacientes con trastornos gastrointestinales funcionales (TGF) y la dificultad para el gastroenterólogo para detectarla y manejarla, pudiendo generar diagnósticos digestivos tardíos y en ocasiones erróneos. Los trastornos de la motilidad gastrointestinal (TMG) han sido menos estudiados desde ésta perspectiva. Tener en cuenta la experiencia subjetiva del paciente en relación a su enfermedad en comparación con la evaluación del gastroenterólogo y el nivel de incongruencia entre los dos puede ser de utilidad en el abordaje de estos pacientes. Objetivos: Estudiar la psicopatología, la calidad de vida y los rasgos de personalidad en una muestra de pacientes ingresados para estudio de la motilidad digestiva en una unidad hospitalaria altamente especializada. Por otro lado, se investigó la incongruencia de la funcionalidad evaluada por clínicos y pacientes y su relación con la psicopatología. Metodología: La presente investigación se describe como un estudio observacional, transversal, de pacientes que ingresan en la unidad de hospitalización digestiva para estudio de la motilidad gastrointestinal. Durante dos años, se incluyeron en este estudio todos los pacientes hospitalizados para la evaluación de la función motora gastrointestinal en una unidad de patología digestiva altamente especializada, de un centro de referencia terciario. Se incluyeron un total de 103 pacientes y fueron evaluados por el psiquiatra en las primeras 72 horas posteriores a la admisión al hospital. Se llevó a cabo una entrevista clínica y se administraron las siguientes escalas autoaplicadas: HADS, SCL-90-R, TAS-20, SF-36 y NEO-FFI-R. A nivel digestivo se realizaron las pruebas de motilidad oportunas así como la administración del Karnofsky Performance Status Scale (KPS) para evaluar la funcionalidad. Gastrointestinal motility disorders (GMDs) have been less studied from this perspective. Taking into account the subjective experience of the patient in relation to their disease, compared to the evaluation of the gastroenterologist and the level of incongruence between the two may be useful in the approach of these patients. Objectives: To study psychopathology, quality of life and personality traits of patients admitted for specialized assessment in an inpatient tertiary Digestive Unit. On the other hand, the incongruence of the functionality evaluated by clinicians and patients and its relation with psychopathology was investigated. Methods: The present research is described as an observational, cross-sectional study. We included all patients hospitalized for evaluation of gastrointestinal motor function in a highly specialized Digestive Unit based at a tertiary referral center during two years. A total of 103 patients were included and evaluated by the psychiatrist within 72 hours of admission to the hospital. A clinical interview was conducted and the following self-administered scales were used: HAD, SCL-90-R, TAS-20, SF-36 and NEO-FFI-R. Digestive diagnostic tests were performed; to evaluate the functionality Karnofsky Performance Status Scale (KPS) was used. The incongruence variable was defined as the difference between KPS and the physical performance subscales of the SF-36 (both in the 0-100 range). Results: There were no remarkable differences between the two groups in relation to sociodemographic data, personality traits or psychopathology. La aplicación de la evidencia científica disponible sobre la depresión mayor (DM) en la práctica clínica es compleja. Numerosos estudios muestran que más del 50% de los pacientes depresivos en Atención Primaria (AP) no reciben atención apropiada, o tratamientos eficaces. El proceso de difusión e implementación de las Guías de Práctica Clínica (GPC) induce cambios modestos en la práctica clínica. Una versión computarizada e integrada en los registros clínicos electrónicos de AP de Cataluña de la adaptación de la GPC de la Depresión Mayor en el Adulto del SNS (GPC-DEPc), ofrece una oportunidad extraordinaria para mejorar los resultados. Objetivos: Se evalúan los efectos de un proceso activo de implementación multifacética de la GPC-DEPc, mediante el análisis de su utilización y de los resultados clave en diagnóstico, adecuación y adherencia al tratamiento antidepresivo (AD), utilización de servicios especializados en Salud Mental y conductas suicidas. Metodología: Se realiza un ensayo aleatorizado por conglomerados, multicéntrico, en diez centros de AP de Barcelona, coordinados con un mismo servicio de salud mental. En 5 centros la GPC-DEPc se implementa activamente y en otros 5 se realiza el proceso habitual y sirven como control. El proceso activo de cuatro meses incluye: formación interactiva, visitas de extensión educativa, auditoría con feedback regular y recordatorios periódicos. Analizamos la utilización de la guía para todos los pacientes con diagnóstico de DM antes de la implementación (T0) y durante seis meses tras inicio implementación (T1). The process of dissemination and implementation of Clinical Practice Guidelines (CPG) induces modest changes in clinical practice. A new computerized and integrated version in the electronic clinical records of PC of Catalonia, of a Major Depression CPG (GPC-DEPc), offers an extraordinary opportunity to improve results. Objectives: To evaluate the effects of an active multifaceted implementation process of the CPG-DEPc, analyzing its use and the key results in diagnosis, adequacy and adherence to antidepressant treatments (AD), utilization of mental health specialized services and suicidal behavior. Methodology: A multicenter cluster randomized trial was carried out in ten PC centers in Barcelona, coordinated by the same mental health service. In 5 centers, the CPG-DEPc is actively implemented and in another 5, the usual diffusion process is performed and serve as a control. The active four-month process includes: interactive training, educational outreach visits, regular feedback audit, and periodic reminders. We analyzed the use of the guide for all patients diagnosed with MD four-month prior to implementation (T0) and for six months after starting the implementation (T1). Changes in the diagnosis, use of antidepressants and specialized services and suicidal behaviors during T0 and T1 are reviewed, comparing the control and active centers. Introducción: La exploración neuropsicológica es esencial en el ámbito de las demencias y útil para realizar el diagnóstico diferencial con otros trastornos. Existe una creciente necesidad en la evaluación de la cognición en pacientes ancianos con datos normativos ajustados por edad y escolaridad. El objetivo de este trabajo es presentar la creación, validación y normalización de una nueva herramienta que permita la evaluación cognitiva en sujetos con riesgo de padecer demencia, llamada BNB-D. Método: Se ha administrado la BNB-D a 1680 sujetos control y 5800 pacientes entre 50 y 93 años de edad. Este estudio proporciona las propiedades psicométricas de esta nueva herramienta en la evaluación cognitiva. Resultados: La validez concurrente entre la BNB-D y el 05SE fue r=62 (Sig. P= 0,001), con el ADAS-Cog r= 0,814 (Sig. P= 0,001), con la R-BANS r= 0,846 (Sig. P= 0,001) y con el Test Barcelona r= 0,805 (Sig. P= 0,001). La consistencia interna mostró una Alfa de Cronbach de 0.687. El índice Kappa utilizado para determinar la fiabilidad inter-evaluador fue de K=0.92. La fiabilidad test-retest fue r= 0,91 (Sig. There is an increasing need for standardized assessment of cognition in older patients with normative data adjusted for age and scholarship. The aim of this paper is to present the creation, validation and normalization of a new tool that allows for an evaluation of the cognitive status in subjects at risk of dementia called BNB-D. Methods: We tested BNB-D in 1680 control subjects and 5800 patients between 50 and 93 years of age. This paper reports the psychometric properties of this new tool in cognitive assessment. The effect on BNB-D by age and educational level was established using lineal regression model. Results: The concurrent validity between the BNB-D and MMSE was r= 0,62 (Sig. P= 0,001), with ADAS-Cog was r= 0,814 (Sig. P= 0,001), with R-BANS was r= 0,846 (Sig. P= 0,001) and with Barcelona Test was r= 0,805 (Sig. P= 0,001). Internal consistency showed a Cronbach Alpha of 0.687. The Kappa index used to determine Inter-rater reliability was of K=0.92. Test-retest reliability was r= 0,91 (Sig. P= 0,001) for Intraclass Correlation Index (ICC). Regression analysis showed clear effect on BNB-D by age and educational level (ANOVA F of regression models with P values less than 0,05). Conclusions: The BNB-D fulfills the basic principles of classic psychometrics of construct criterion, validity and reliability. Existe una gran brecha entre la observación clínica y científica del papel del trastorno por estrés post-traumático (TEPT) y los acontecimientos traumáticos en la vida de paciente con diagnóstico de trastorno bipolar y la casi ausencia de guías de tratamiento para esta comorbilidad. En la práctica clínica del tratamiento del trastorno bipolar hay una ausencia importante de ensayos clínicos en terapias focalizadas en el trauma de pacientes bipolares. Dada la importancia del TEPT, los eventos traumáticos y los eventos vitales en el pronóstico y la evolución en pacientes con trastorno bipolar se deberán investigar y proporcionar las intervenciones basadas en la evidencia del trauma en el trastorno bipolar. El tratamiento psicoterapéutico "Desensibilización y reprocesamiento por movimiento ocular", más conocido por sus siglas en inglés EMDR ("Eye movement desensitization and reprocessing") es un tratamiento psicológico desarrollado por Francine Shapiro y actualmente está indicado para el tratamiento del TEPT por distintas organizaciones internacionales (American Psychological Association Presidential Task Force on Evidence-Based Practice, 2006; National Institute for Clinical Excellence, 2005; Ursano y cols., 2004; World Health Organisation, 2013). Sin embargo, existen indicios de que se trata de dos poblaciones diferenciadas. Esto implica que las estrategias preventivas más centradas en los intentos de suicidio, podrían no ser suficientes para prevenir el suicidio consumado. Por otro lado, sabemos que solo una proporción pequeña de aquellas personas que presentan tentativas acabaran consumando el suicidio. El tipificar mejor la población de personas que presentan riesgo de suicidio permitiría iniciar estrategias preventivas más eficaces. Objetivos: Comparar las características sociodemográficas y clínicas entre una población que ha realizado intentos de suicidio y otra de suicidios consumados, y el seguimiento de las conductas suicidas durante un periodo de 5 años para identificar mejor a la población con mayor riesgo de reintentar o consumar el suicidio. Metodología: Se seleccionaron todos los casos de tentativas autolíticas atendidas en el servicio de urgencias del Hospital Parc Taulí de Sabadell durante un año natural. La información se obtuvo a partir de la información registrada en la historia clínica hospitalaria, de primaria y de los informes clínicos de urgencias. También, se evaluaron todos los suicidios consumados durante un periodo de 3 años, recabando la información facilitada por el forense. However, there are indications that these are two distinct populations. This implies that preventive strategies more focused on attempted suicides may not be sufficient to prevent completed suicide. On the other hand, we know that only a small proportion of those who have attempted will complete suicide. Better characterization of the population of persons at risk of suicide would allow more effective preventive strategies to be initiated. Objectives: To compare sociodemographic and clinical characteristics between a population of suicide attempts and another of completed suicides, and to follow up the suicide attempts during a period of 5 years to better identify the population at greater risk of re-attempting or consuming suicide. Methodology: All suicide attempts attended at the emergency unit of the Parc Taulí Hospital in Sabadell during a calendar year were selected. The information was obtained from the information recorded in the hospital history, primary and emergency clinical reports. We also evaluated all completed suicides over a period of 3 years, and also obtained information provided by the coroner. In order to carry out the second part of the project, all the suicide attempts attended in the emergency department and all completed suicides committed until December 31, 2012, were registered. En aquesta Tesi es presenten quatre estudis que tenien l'objectiu de validar les rates romanes (RHA-I i RLA-I) com a model animal d'alguns símptomes rellevants per a l'esquizofrènia. L'Estudi 1 tenia com a objectiu principal fer la caracterització conductual de les rates romanes d'alta i baixa evitació (RHA-I i RLA-I, respectivament) i de les rates genèticament heterogènies NIH-HS en dues tasques relacionades amb els símptomes de l'esquizofrènia, com són els dèficits de filtratge atencional, mesurats amb la inhibició de la resposta de sobresalt per un prepols (PPI), i els dèficits de memòria de treball. A l'Estudi 2 es van investigar els efectes de l'aïllament social, sobre diverses conductes/fenotips: el filtratge atencional (PPI), l'ansietat, la hiperactivitat motora i la memòria. A l'Estudi 3 es van avaluar els efectes de substàncies antipsicòtiques (haloperidol i clozapina) i propsicòtiques (apomorfina, MK-801, DOI) sobre la PPI i l'activitat motora. Finalment, el quart estudi tenia l'objectiu d'estudiar les diferències d'expressió gènica de HTR1A i HTR2A entre les soques romanes, i entre rates NIH-HS seleccionades pel seus nivells extrems de PPI. Els resultats de l'Estudi 1 mostren que les rates RHA-I, comparades amb les RLA-I i les NIH-HS, tenen dèficits de PPI i de memòria de treball, i que les rates NIH-HS presenten valors alts (semblants als de les rates RLA-I) tant de PPI com de memòria de treball. A més a més, els resultats mostren correlacions positives entre la PPI i la memòria de treball. Study 1 was devoted to behaviorally characterize the Roman high- (RHA-I) and low-avoidance (RLA-I) rats, and the genetically heterogeneous rats (NIH-HS), in two tasks related to some schizophrenia-relevant symptoms, i.e. prepulse inhibition of the startle response (PPI, a measure of sensorimotor gating) and spatial working memory measured in the Morris water maze. In Study 2, we investigated the effects of chronic social isolation on several behaviours/phenotypes in the Roman rats: sensorimotor gating (PPI), anxiety, locomotor hyperactivity and spatial memory. In Study 3 we evaluated the effects of several antipsychotics (haloperidol and clozapine) and propsychotics (apomorphine, DOI and MK-801) on PPI and locomotor activity. Finally, Study 4 was aimed to evaluate whether there are differences in gene expression (of the HTR1A and HTR2A genes) between the Roman rat strains, and to see if these differences also appear between NIH-HS rats selected for their extremely high or low levels of PPI. The results of Study 1 show that RHA-I rats have PPI and working memory deficits and that NIH-HS rats present high levels of PPI and working memory. There are positive associations between PPI and working memory, since rats with high PPI (RLA-I and NIH-HS) also have a better performance in the working memory task. In Study 2, the socially-isolated RHA-I rats exhibit PPI deficits, increased anxiety, locomotor hyperactivity and spatial learning deficits. Study 3 shows that RHA-I rats are more sensitive to the effects of dopamine agonists/antagonists (apomorphine/haloperidol) on PPI, whereas RLA-I rats are more sensitive to the effects of serotonin agonists/antagonists (DOI/clozapine) and to the NMDA antagonist (MK-801) in the PPI test. La presente Tesis Doctoral presenta una serie de estudios centrados en los efectos de la manipulación neonatal temprana (NH, "neonatal handling") sobre respuestas de ansiedad/estrés y atencionales-cognitivas. Todos los estudios se llevaron a cabo utilizando las cepas de ratas Romanas consanguíneas, que han sido seleccionadas bi-direccionalmente por su excelente (RHA) vs extremadamente pobre (RLA) adquisición de la respuesta de evitación activa en dos sentidos. La cepa de ratas RLA es más ansiosa/temerosa que las hypoemocionales RHA, quienes a su vez presentan mayor impulsividad y peor rendimiento atencional-cognitivo. Nuestro trabajo está dirigido a dos objetivos principales: (i) evaluar posibles efectos positivos del NH en respuestas de ansiedad/estrés y en procesos atencionales/cognitivos en ambas cepas de ratas. (ii) evaluar diferencias volumétricas (resonancia magnética estructural) entre ambas cepas de ratas en regiones cerebrales relacionadas con la ansiedad, el estrés y procesos cognitivos, así como los efectos a largo plazo del NH sobre tales procesos y áreas cerebrales. Se realizaron tres estudios: los estudios 1 y 2 evalúan los efectos del NH sobre respuestas de ansiedad y respuestas hormonales post-estrés, así como sobre los volúmenes de regiones cerebrales relacionadas con la ansiedad o el estrés (hipocampo, amígdala). En el estudio 3 se evaluaron los efectos del NH sobre el filtraje atencional (PPI) y funciones ejecutivas (memoria/aprendizaje espacial y memoria de trabajo) en ambas cepas de ratas. Se incluyeron también medidas de volúmenes de áreas cerebrales asociadas a dichas tareas atencionales/cognitivas (córtex prefrontal medial, córtex cingulado anterior, núcleo accumbens, ventrículos laterales). Los resultados indican que: i) el NH reduce la ansiedad y las respuestas hormonales al estrés en ambas cepas de ratas, especialmente en las RLAs, mientras que también mejora los niveles de PPI y la memoria de trabajo en las ratas RHA; ii) respecto a las r, The present Doctoral Thesis reports a series of studies dealing with the effects of neonatal handling (NH) treatment on anxiety/stress and attentional-cognitive processes. All the studies were performed using the inbred Roman rat strains, which have been bi-directionally selected by their very good (RHA) vs extremely poor (RLA) acquisition of the two-way active avoidance response in the shuttle-box task. RLA rats are more anxious/fearful than their hypoemotional RHA counterparts, which in turn display higher impulsivity and worse attentional/cognitive performance than the former. Our work was aimed to address two main objectives: First, to evaluate the potential NH-improving effects on both, the anxiety/stress and attentional/cognitive profiles of both strains of rats. Secondly, to evaluate regional volumetric differences (using structural MRI) between both rat strains in brain areas related to anxiety, stress and cognition, as well as the eventual long-lasting effects of NH on anxiety- and attention/cognition-related brain regions. To this aim, three studies were conducted: Study 1 and Study 2 were mainly focused on anxiety/stress responses. Thus, we evaluated the effects of NH on anxiety/fear phenotypes, on post-stress hormonal responses and on the volume of specific brain areas involved in anxiety/stress processes (e.g. hippocampus, amygdala). Study 3 was aimed at evaluating some attentional/cognitive processes in RHA and RLA rats, as well as the potential NH effects on sensorimotor gating (prepulse inhibition, PPI) and executive functions (spatial learning/memory and working memory) in both rat strains. Así pues, tiene sentido que se preste atención a la influencia que estos animales ejercen en la sociedad, desde los problemas generados por la convivencia con animales de compañía, hasta los beneficios que aportan a nivel individual y social. Y es la antrozoología la ciencia que esta relación entre humanos y el resto de animales. Mediante esta tesis se pretende, en primer lugar, a partir de muestras de población española, añadir investigación en el ámbito de la antrozoología internacional, ampliando el conocimiento de cómo es a nivel psicosocial la relación o vínculo con los animales de compañía, mediante el estudio de estas relaciones desde tres ángulos diferentes y complementarios: -La existencia de un vínculo humano-animal en la convivencia rutinaria entre un animal de compañía y el ser humano adulto que es su propietario. -El aprovechamiento consciente de este vínculo humano-animal, mediante la aplicación terapéutica del mismo. -La existencia de un vínculo humano-animal disfuncional en determinados individuos humanos. El objetivo general de esta tesis doctoral es profundizar en la relación que se establece entre el ser humano y los animales de compañía, tanto en su vertiente positiva como negativa. Para estudiar el vínculo humano-animal en la convivencia con los animales de compañía, se llevó a cabo un estudio con una muestra de conveniencia de participantes voluntarios propietarios de perros en España. El objetivo era explorar la existencia de diferentes patrones de relación de las personas con sus perros y los factores y/o variables que pudieran contribuir en los diferentes patrones de relación propietario-perro, desde el punto de vista de la percepción del propietario. Tras recopilar 1140 respuestas y analizar los resultados, encontramos dos patrones diferenciados de propietarios de perro (según su relación propietario-perro), un tipo emocional y otro pragmático. We tried to extend knowledge of the psychosocial aspects of the relationship or bond between human and companion animals, through studying these relationships from three different but complementary aspects: - The existence of a human-animal bond in the routine cohabitation between companion animals and adult human beings. - The conscious utilization of the human-animal bond for therapeutic purposes. - The existence of some human individuals presenting dysfunctional human-animal bond. The global objective of this thesis consists of going further into the issue of the relationships between human beings and companion animals, thouching negative and positive aspects. With the aim of studying human-animal bond in the area of cohabitation between companion animals and their adult human owners, we conducted a study with a convenience sample of volunteer Spanish dog owners. Our objective consisted of, first, exploring the existence of different patterns of owner-dog relationship, and, second, trying to find out the factors and/or variables that could contribute to different patters of owner-dog relationship. We could compile 1140 complete questionnaires. After analysing all data, we found two different patterns of dog owners (from the owner's perspective): a more emotional dog owner profile and a more pragmatic dog owner pattern. Los objetivos principales de esta tesis son validar instrumentos de medida de mindfulness a nuestro contexto cultural y posteriormente poder evaluar los niveles de mindfulness en muestras de pacientes psiquiátricos en comparación con sujetos control. Esta tesis doctoral se compone de tres estudios: el primer estudio tiene como objetivo validar al español la M17S (Brown & Ryan, 2003), la escala más utilizada en estudios de investigación de mindfulness (Sauer et al., 2013) y centrada en el componente atencional del mindfulness. El segundo estudio, se centra en validar al español la PHLMS (Cardaciotto, Herbert, Forman, Moitra, & Farrow, 2008), una escala bidimensional que valora el mindfulness con dos componentes uno centrado en la atención/conciencia y otro en la actitud de aceptación. Finalmente, el tercer estudio se dirige a examinar el papel de los dos componentes del mindfulness en una muestra control no psiquiátrica y en diferentes submuestras psiquiátricas como son pacientes con diagnóstico de trastorno límite de la personalidad, trastorno por dependencia de cocaína, trastorno de la conducta alimentaria y trastorno depresivo mayor recurrente en remisión. This doctoral thesis consists of three studies: the first study aims to validate to Spanish version of MAAS (Brown & Ryan, 2003), the most used scale in research studies of mindfulness (Sauer et al., 2013) and the MAAS is scale focused on the attentional component of mindfulness. The second study, aims to validate to Spanish version of PHLMS (Cardaciotto, Herbert, Forman, Moitra, & Farrow, 2008), a two-dimensional scale that asses mindfulness with two components, one focused on the attention/awareness and another in the acceptance. Finally, the third study is aimed at examining the role of the two components of mindfulness in a non-psychiatric control sample and in different psychiatric subsamples, including patients diagnosed with borderline personality disorder, cocaine dependence disorder, eating disorder, and recurrent major depressive disorder in remission. For this purpose, the mindfulness scales previously validated in Spanish are used. Our results suggest that the MAAS and the PHLMS Spanish version allow the measurement of the capacity of mindfulness from a unifactorial and bifactorial perspective respectively and obtain a good reliability and validity. In relation to the two components of mindfulness, our results suggest that there is impairment on acceptance in most of the psychiatric sample and that this impairment is not dependent to mood. This could indicate that the deficit in this component could be a key transdiagnostic element. Objetivos: En el TDAH existen déficits en las Funciones Ejecutivas (FFEE) y en especial en la Memoria de trabajo (MT), que repercuten negativamente en el rendimiento académico, los síntomas clínicos, y la adaptación funcional. Los objetivos de este estudio fueron analizar la eficacia de un entrenamiento computarizado en MT sobre otras FFEE frías y calientes, aprendizajes, síntomas clínicos y adaptación funcional. Método: Se incluyeron 66 niños con TDAH tipo combinado entre 7 y 12 años de edad de una Unidad de Psiquiatría Infantil y Adolescente en este ensayo clínico controlado con placebo, a doble ciego y con asignación aleatoria a realizar un entrenamiento computarizado en MT (RoboMemo®, Cogmed Working Memory Training™) (n=36) o un entrenamiento placebo (n=30). Se realizaron evaluaciones en la línea base, entre 1-2 semanas post intervención y 6 meses post intervención administrando escalas y pruebas cognitivas de FFEE frías y calientes, y cuestionarios de síntomas clínicos y de adaptación funcional. Los participantes, sus familias, profesores y los profesionales que realizaron las evaluaciones cognitivas fueron ciegos. The aims of the present study were to analyse the efficacy of a computerised WM training on other cool and hot EF, learning, clinical symptoms, and functional impairment. Method: 66 children with combined-type ADHD between 7-12 years of age from a Child and Adolescent Psychiatric Unit were included in this randomized, double-blind, placebo-controlled, parallel-group clinical trial. The participants were randomized (1:1) to an experimental group (RoboMemo®, Cogmed Working Memory Training™) (n=36) or a control group (placebo training). Assessments were conducted at baseline (T0), 1-2 weeks (T1), and 6 months post-intervention (T2) with the administration of EF rating scales, performance-based measures of cool and hot EF, measures of academic achievement, and questionnaires regarding clinical symptoms and functional impairment. The participants, their parents, their teachers and professionals who performed the cognitive assessments were blinded. Correlational analysis between WM and hot EFs at baseline were also performed to better clarify the nature of this interrelation. Results: Adjusted multiple linear regression analysis showed significant improvements in cool EF scales, especially at long-term, in performance-based measurements of cool EFs response inhibition and sustained attention at short-term (that persisted at long-term), and in ADHD symptoms and functional impairment in school environment at long-term. Effect sizes were mostly moderate to large. Statistically significant correlations were found between WM and Theory of mind (ToM) measures at baseline, but not between WM and decision-making. Tales alteraciones pueden modular la expresión de la enfermedad con implicaciones sobre su gravedad, evolución y pronóstico. Estudios previos han mostrado la influencia de la función y autoinmunidad tiroideas en la evolución de la enfermedad en pacientes psicóticos y bipolares. Sin embargo, el mecanismo de acción de esta modulación no ha sido aún esclarecida. Los estudios que investigan la relación entre los niveles de hormonas tiroideas y el estado de la autoinmunidad tiroidea y las funciones cognitivas, han encontrado resultados inconsistentes y contradictorios. Such alterations may modulate the expression of the illness with implications on its severity and prognosis. Previous studies show the influence of the function and thyroid autoimmunity on the evolution of the disease in bipolar and psychotic patients. However, the mechanism of action of this modulation has not been clarified yet. Studies searching the relationship between thyroid hormones levels and thyroid autoimmunity and cognitive functions have found contradictory and inconsistent results. Cognitive impairment has a major impact on the functionality of psychotic patients and the overall prognosis of the disease. We aimed to clarify the role of thyroid function and autoimmunity on the modulation of the evolution of affective and non-affective psychotic disorders in three aspects; severity of symptoms, risk of relapse and cognitive impairment. We also studied if this modulation is different in affective and non-affective psychoses. For this purpose we studied three patient samples. One sample of bipolar patients (N=77) requiring hospitalization for a manic or mixed episode that were followed up for one year to study the risk of readmission. For the study of the relationship between thyroid function and autoimmunity and cognitive abilities, two different samples were used: one sample of 70 patients with an early psychosis (cross-sectional study including the differentiation of the effects on cognition in affective and non-affective psychoses) and a sample of 36 patients with a first psychotic episode (one year longitudinal study). L'esquizofrènia és un trastorn psiquiàtric caracteritzat per la presència de deliris i al·lucinacions a banda d'altres símptomes de tipus emocional i cognitiu, que sembla tenir una base causal relacionada amb el neurodesenvolupament. A nivell fenomenològic s'ha pogut caracteritzar l'esquizofrènia a través dels símptomes observables, que són descrits com a simptomatologia positiva, negativa i cognitiva, representant una àmplia heterogeneïtat en la seva presentació clínica. Fins ara hi hagut diferents intents de definir subgrups específics dins del trastorn per tal d'aconseguir una caracterització aproximada en termes d'etiologia i fisiopatogènia i del possible substrat anatòmic, i aquestes caracteritzacions han estat bàsicament sempre realitzades a partir d'aquests grans grups sindròmics però no a través dels símptomes per separat. És possible que l'estudi individualitzat dels diferents símptomes presents en el trastorn puguin aportar també una informació vàlida pel que fa al seu correlat fisiopatogènic i anatòmic cerebral, donant peu a un coneixement més ampli dels mecanismes etiopatogènics del trastorn. Resultats: Després d'una anàlisi factorial del contingut dels deliris, s'observa que aquests s'agrupen en tres, Schizophrenia is a psychiatric disorder characterized by the presence of delusions and hallucinations, in addition to other emotional and cognitive symptoms, and it seems to have a causal basis related to the neurodevelopment. At a phenomenological level, schizophrenia has been characterized using clinical symptoms, constituting what has been designated as positive, negative and cognitive symptomatology. These syndromic groups however, have a very heterogeneous clinical presentation. To date, several attempts to classify schizophrenia into specific subgroups have been performed with the aim of obtaining a rough classification in terms of etiology, physiopathology, and anatomy. However, these classifications have always been performed using the three big syndromic groups, and a classification using individual symptoms has been never attempted. The study of specific symptoms may shed some light on the physiopathogenic and anatomic brain correlates, which may lead to a broader knowledge on the etiological mechanisms of schizophrenia. Objective: to use the contents of the delusional symptomatology of a sample of patients with schizophrenia to characterize their phenomenological phenotypes and to study the clinical characteristics and evolution, as well as the existence and characteristics of a possible specific neuropsychological correlate. Methods: Six-month prospective cohort study using a sample of patients with a diagnosis of schizophrenia hospitalized in a Psychiatric Unit. Sociodemographic information was collected, as well as clinical information on the severity of psychotic symptoms, and on the contents of delusional symptoms. El objetivo principal de la tesis fue evaluar, en estudios con animales y humanos, el impacto que tiene la práctica de ejercicio físico sobre variables conductuales, metabólicas y nutricionales, considerando diferencias de género en individuos de peso normal o con sobrepeso/obesidad. En los estudios con animales, se usaron ratas macho y hembra adultas de peso normal o con obesidad inducida por dieta (OID). Las ratas fueron entrenadas a una intensidad moderada (12 metros/minutos) y más alta (16 metros/minutos) durante 30 minutos, 4-5 días a la semana, tomando como referencia las recomendaciones de actividad física de la Organización Mundial de la Salud (OMS) y adaptándolas al modelo animal. Se evaluaron variables biométricas, plasmáticas, neuroendocrinas, de sintomatología ansiosa y estrategias de afrontamiento. En los estudios con humanos, se analizó el rendimiento académico, la condición física, el estado nutricional y parámetros psicológicos relacionados con la salud mental en estudiantes de enseñanza básica, secundaria y universitaria. Los estudios en animales indicaron que las ratas hembras aprendieron mejor y más rápidamente que los machos la evitación activa de dos sentidos (SB), en términos de mayor número de respuestas de evitación, latencias de escape más cortas y menor número de ensayos hasta alcanzar el criterio de aprendizaje. En ambos sexos, el ejercicio mejoró la ejecución de la tarea, al aumentar el número total de evitaciones y disminuir el número de ensayos necesarios hasta alcanzar el criterio de aprendizaje. Las ratas obesas alimentadas con dieta cafetería (CAF) presentaban un aumento de masa y un conjunto de alteraciones metabólicas, como un aumento de los niveles plasmáticos de glucosa, insulina, triglicéridos y leptina, así como una disminución de la sensibilidad a la insulina, con respecto a las ratas de peso normal. El ejercicio disminuyó el porcentaje de masa grasa retroperitoneal y los niveles de leptina en las ratas con dieta CAF. In the animal studies, male and female adult rats with normal weight or with diet-induced obesity (DIO) were used. The rats were trained on a treadmill at moderate intensity (12 m / min) and high intensity (16 m / min) for 30 minutes, 4-5 days week, taking as reference the WHO Global recommendations on physical activity for health and adapting to the animal model. Biometric, plasma, neuroendocrine variables, anxiety-related symptoms and stress-copy strategies were evaluated. In the human studies, academic performance, physical fitness, nutritional status and psychological parameters related to mental health of primary, secondary and university students were assessed. Animal studies indicated that female learned better and faster that male for two-way activity avoidance learning (SB), in terms of higher number of avoidance responses, shorter escape latencies and fewer trials to reach performance criteria. In both sexes, exercise improved task performance by increasing the total number of avoidances and reduce the number of trials needed to reach the performance criteria. The rats that were fed with cafeteria diet (CAF) show an increase in body weight and a set of metabolic disorders such as retroperitoneal fat percentage, glucose, insulin, triglycerides, and leptin plasma levels as well as a decrease in insulin sensitivity regarding to normal weight rats. Los modelos dimensionales de personalidad normal y los modelos dimensionales de personalidad patológica han experimentado un rápido nivel de desarrollo e integración en los últimos 20 años. Sin embargo, existen en la literatura algunas cuestiones importantes por resolver. Una de las más importantes es cómo y en qué grado la personalidad influye en que tengamos o no dificultades para adaptarnos con éxito al entorno en los diferentes ámbitos de la vida, tales como alcanzar un nivel de estudios, conseguir y mantener un trabajo, conseguir ingresos suficientes, formar una red de apoyo social, tener pareja e hijos, adaptarnos a las normas, o preservar nuestra salud. Dado que todos estos ámbitos son una carrera de obstáculos, otra cuestión importante es cómo la personalidad determina las diferentes maneras de desenvolverse frente a los problemas y adversidades. Ambos son aspectos nucleares al concepto de adaptación humana, y en la conceptualización y evaluación de la patología de la personalidad. En la presente tesis se pretende dar respuesta a estas cuestiones estudiando de qué modo la personalidad, de acuerdo al modelo de temperamento y carácter de Cloninger (1994), influye en los más importantes ámbitos de nuestra vida (p.e. estudios, trabajo, relaciones, psicopatología, malestar, discapacidad, etc.), y de qué manera predice el modo en que las personas afrontamos las dificultades y el estrés Para la consecución de este objetivo, se realizaron dos estudios. However, there are a few important questions left unresolved in the literature. One of the most important questions is how personality influences life adaptation in different domains, as achieving a certain academic degree, obtaining and maintaining a job, getting enough incomes to cover needs, forming a social network, having couple and offspring, adapt to social norms, or maintaining health. Given that life is like a steeplechase, another important question is how personality determines the different ways of coping with problems and adversity. Both of these questions represent nuclear aspects of the human adaptation concept, and for the representation and evaluation of personality pathology. This thesis aims at answering these questions studying how personality, according to Cloninger's temperament and character model, affects most important life domains (e.g. education, career, relationships, psychopathology, distress, disability, etc.), and how personality predicts the ways people cope with stress and difficulties. Two independent studies were performed in order to accomplish these aims. In the first study, we investigate how personality assessed with the Temperament and Character Inventory-Revised is related with a wide variety of objective and subjective indicators of adjustment assessed with the Life Outcome Questionnaire. In the second study, we investigate how personality assessed with the Temperament and Character Inventory relates to different coping strategies as assessed with the Coping Orientation to Problems Experienced. Antecedentes y objetivos: el tempo cognitivo lento (TCL) se ha asociado, desde su definición, al trastorno por déficit de atención con hiperactividad subtipo inatento (TDAH-I). Se han hallado síntomas TCL en el trastorno por déficit de atención con hiperactividad subtipo combinado (TDAH-C), en otros trastornos psiquiátricos y en grupos clínicos sin trastorno mental. Existen correlaciones entre los síntomas de TCL y síntomas de ansiedad, depresión, dificultades de regulación emocional de emociones negativas, mayor sensibilidad al castigo, evitación social, bajo rendimiento académico y con un peor funcionamiento social. La validez estadística de los síntomas TCL ha quedado demostrada diferenciándose del TDAH y presentando unas características propias. Los estudios de población general sugieren que probablemente los síntomas TCL presenten diferentes correlatos socio-demográficos, asociación con determinados déficits de funciones ejecutivas y diferentes ámbitos de discapacidad psicosocial en relación al TDAH. Los objetivos de esta tesis son: realizar una revisión del concepto TCL, determinar su prevalencia, características clínicas y sociodemográficas en la población general y clínica infanto-juvenil, estudiar la relación entre TDAH y TCL y finalmente delimitar el perfil neuropsicológico específico para los síntomas TCL. Métodos: para llevar a cabo los objetivo se han realizado 4 estudios. El primero, se trata de una revisión sistemática de la literatura. Para determinar las prevalencias, características clínicas, y conductuales tanto en población general infanto-juvenil como clínica se realizaron el segundo y tercer estudio, respectivamente. Y, finalmente, el cuarto estudio, en la muestra de población general infantil, se estudió el perfil neuropsicológico asociado a los síntomas TCL. Resultados: se han hallado síntomas TCL tanto en población general como clínica, duplicándose en esta última. Se asocian a genero masculino, a población clínica y a mayor edad. La exposic, Background and objectives: Sluggish cognitive tempo (SCT) has been associated, ever since it was first defined, with attention deficit hyperactive disorder, inattentive subtype (ADHD-I). SCT symptoms have been observed in ADHD, combined subtype (ADHD-C), in other psychiatric disorders and in clinical groups with no mental disorder. There are correlations between the symptoms of SCT and symptoms of anxiety disorders, depression, problems controlling negative emotions, heightened sensitivity to punishment, social disengagement, low academic achievement and with impaired social functioning. The statistical validity of SCT symptoms has been demonstrated through their differentiation from ADHD and the presenting of specific characteristics. Studies carried out among the general population (both in adults and in children) suggest that SCT symptoms present different socio-demographic correlates, association with specific executive function deficits and different areas of psychosocial disability related to ADHD. The objectives of this doctoral thesis are: to review the concept of SCT; to determine the prevalence of SCT, its clinical and sociodemographic characteristics in the general population and in the infant-adolescent clinical population; to study the relation between ADHD and SCT; and finally, to delimit the specific neuropsychological profile of SCT symptoms. Methods: In order to achieve those objectives, I have carried out four studies as part of an active research group. The first was a systematic review of the literature. The next two studies aimed to determine the prevalence, and the clinical and behavioural characteristics of SCT in both the general (second study) and clinical (third study) infant-adolescent populations. Introducción: El Trastorno de pánico (TP) es una de las enfermedades psiquiátricas más comunes, con un coste económico y social importante. El uso de Internet para la detección y tratamiento del TP permite llegar a un gran número de personas, en amplias zonas geográficas y de forma confidencial. Además, pueden servir como autoayuda o como complemento de la terapia tradicional. Los instrumentos para la detección del TP online pueden ser muy útiles, sin embargo, actualmente son escasos. Por otro lado, en diversos países se están probando programas de tratamiento a través de Internet para el TP y los resultados son prometedores. Se ha observado que el efecto es mayor cuando se añade apoyo psicológico, pero no está claro cómo éste debe administrarse. En esta tesis se abordan la detección y el tratamiento online del TP por medio de dos estudios. En el estudio 1 se analizó la validez de un ítem para detectar la presencia de síntomas de TP extraído del Web Screening Questionnaire, el WSQ-Panic. En el estudio 2 se analizaron dos formas de administrar el apoyo psicológico como parte de un tratamiento de autoayuda a través de Internet basado en la terapia cognitivo conductual llamado Libre de Ansiedad. Estudio 1 Método: Un total de 171 sujetos completaron el WSQ-Panic a través de Internet y fueron evaluados por teléfono mediante la Entrevista Clínica Estructurada para los Trastornos del eje I del DSM-IV (SCID-I), que se empleó como gold standard. Se calculó la sensibilidad, la especificidad, los valores predictivos positivos (VPP) y negativos (VPN), el área bajo la curva ROC y se determinó el punto de corte óptimo. Resultados: El WSQ-Panic mostró una sensibilidad de 0,83 y una especificidad de 0,74. El VPP fue de 0,46 y el VPN fue de 0,94. El área bajo la curva ROC fue de 0,83, 95% CI [0,74;0,90], lo cual indica una precisión moderada. El punto de corte óptimo del instrumento fue ≥ 2. Using Internet-based tools to detect and treat PD allows to reach a greater number of people, regardless of their geographical location, and with high confidentiality. Moreover, web-based treatments could be used as self-help tools or as a complement to traditional therapy. Web-based screening tools could be useful, however, there is a paucity of instruments that specifically measure PD online. On the other hand, in some countries Web-based self-help treatments for PD have been tested obtaining promising results. Larger effect sizes have been observed when professional support has been added, however, it is still not clear how this support should be administered. In this thesis the detection and treatment online for PD is addressed through two studies. In study 1, the validity of one item from the Web Screening Questionnaire designed to detect PD symptoms (WSQ-Panic) was analyzed. In study 2, two ways of administering psychological support as a part of Internet-based self-help treatment for PD based on CBT were compared using a program called Free from Anxiety. Study 1 Methods: A total of 171 participants completed the WSQ-Panic online and were assessed by telephone using the Structured Clinical Interview for DSM Disorders (SCID-I), that was used as a gold standard. The sensitivity, the specificity, positive and negative predictive values (PPV, NPV), and area under the ROC curve were calculated, and the optimal cut-off point was determined. Results: The WSQ-Panic showed a sensitivity of 0.83 and a specificity of 0.74. The PPV was 0.46 and NPV was 0.94. The area under the curve was 0.82 (95% CI: 0.74 – 0.90), which indicates a moderate accuracy. The optimal cut-off point is ≥ 2. Los objetivos principales de esta tesis fueron estudiar la disregulación emocional y los déficits en mindfulness en pacientes con trastorno límite de la personalidad (TLP) y explorar los efectos de una intervención en mindfulness sobre la clínica característica del trastorno. Para ello se realizaron cuatro estudios (dos de caracterización del trastorno y dos de intervención en mindfulness). En el Estudio 1 se encontró que los pacientes con TLP, en comparación con controles sanos (CS), presentaron una mayor intensidad basal de emociones negativas, pero no una mayor reactividad emocional ante estímulos inductores de emociones discretas (ira, miedo, asco, tristeza). Sin embargo, sí se observaron diferencias entre grupos (TLP vs. CS) en relación a los estímulos relacionados con la clínica del trastorno (abandono, rechazo y dependencia emocional), para los cuales se encontró una reactividad emocional mayor a nivel subjetivo (pero no fisiológico) en el grupo de TLP. Resultados del Estudio 2 sugieren que algunas experiencias traumáticas infantiles, especialmente, el hecho de haber sufrido abuso sexual, se relacionan con déficits en algunas facetas del mindfulness (i.e., "actuación consciente" y "no-juzgar"). A pesar de esta asociación, parecería ser qué los rasgos temperamentales, especialmente el neuroticismo y la impulsividad, tienen una mayor influencia sobre los déficits en mindfulness que las experiencias traumáticas infantiles. En conjunto, los re, The aims of this dissertation were twofold. The first objective was to study emotional dysregulation and mindfulness deficits in borderline personality disorder (BPD) and the second was to explore the effects of a mindfulness-based intervention on the core characteristics of BPD. To achieve those aims, four studies were carried out (two of them were dedicated to explore the characteristics of the disorder and the other two were focused on investigating the impact of mindfulness training). Findings of Study 1 showed that BPD patients – when compared to healthy controls (HC) – presented more intense negative emotions at baseline. BPD subjects presented no subjective heightened reactivity to film-fragments eliciting discrete emotions (anger, fear, disgust and sadness). However, differences between groups (BPD vs. HC) were observed in regard to BPD-related stimulus (abandonment, rejection and emotional dependence), BPD participants exhibited a more heightened subjective reactivity (but not in physiological variables) than HC. Results from Study 2 suggested that early traumatic experiences, especially sexual abuse, are related to deficits in some mindfulness facets (i.e., "acting with awareness" and "non-judging") in BPD participants. In spite of this association, the results of this study seem to indicate that temperamental traits, specifically neuroticism and impulsivity, have a greater impact on mindfulness deficits than traumatic experiences. Finally, Studies 3 and 4 indicate that a 10-week mindfulness training is efficacious for diminishing BPD's severity and increasing decentering and other mindfulness facets, when compared to a control intervention (i.e., interpersonal effectiveness training). In addition, mindfulness training seems to have a positive impact on some impulsivity-related variables (Study 4), increasing the tolerance for delayed rewards and modifying temporal perception.
