The diagnosis of gastrointestinal stromal tumor (GIST) using conventional endoscopy is difficult because submucosal tumor (SMT) lesions like GIST are covered by a mucosal layer. Near-infrared hyperspectral imaging (NIR-HSI) can obtain optical information from deep inside tissues. However, far less progress has been made in the development of techniques for distinguishing deep lesions like GIST. This study aimed to investigate whether NIR-HSI is suitable for distinguishing deep SMT lesions. In this study, 12 gastric GIST lesions were surgically resected and imaged with an NIR hyperspectral camera from the aspect of the mucosal surface. Thus, the images were obtained ex-vivo. The site of the GIST was defined by a pathologist using the NIR image to prepare training data for normal and GIST regions. A machine learning algorithm, support vector machine, was then used to predict normal and GIST regions. Results were displayed using color-coded regions. Although 7 specimens had a mucosal layer (thickness 0.4-2.5 mm) covering the GIST lesion, NIR-HSI analysis by machine learning showed normal and GIST regions as color-coded areas. The specificity, sensitivity, and accuracy of the results were 73.0%, 91.3%, and 86.1%, respectively. The study suggests that NIR-HSI analysis may potentially help distinguish deep lesions.

Background Recently, atmospheric low-temperature plasma (LTP) has attracted attention as a novel medical tool that might be useful for achieving hemostasis. However, conventional plasma sources are too big for use with endoscopes, and the efficacy of LTP for achieving hemostasis in cases of gastrointestinal bleeding is difficult to investigate. In this study, to solve the problem, we developed a 3D-printed LTP jet that has a diameter of 2.8 mm and metal body for endoscopic use. The characteristics, hemostasis efficacy, and safety were investigated. Materials and methods On investigating the basic characteristics of the developed plasma jet, the electron densities, gas temperatures, and reactive species were measured by emission spectroscopy and thermocouple. To evaluate the efficacy of such hemostatic treatment, porcine gastrointestinal bleeding was treated with the device. In addition, to investigate the safety of such treatment, the CO2 LTP-treated tissue was compared with tissue that was treated with clipping-based or argon plasma coagulation–based hemostasis for 5 d, and hematoxylin and eosin staining was used to evaluate tissue damage in the treated regions. Results The measurement of emission spectroscopy, power, and electron density of various gas plasmas suggested that a high-density (1014 cm−3) LTP of CO2 was generated by the LTP jet, and the gas temperature was 41.5°C at 3 mm from the outlet of the LTP jet. The CO2 LTP achieved hemostasis of oozing blood by 70 ± 20 s. In addition, the CO2 LTP resulted in earlier recovery than clipping-based or argon plasma coagulation–based hemostases, and the treated regions had no damage by the CO2 LTP treatment. Conclusions These results indicated that the developed LTP plasma jet has the potential to be used for endoscopic hemostasis.

A pulsed laser cause vaporization of tissue by plasma if a laser can provide high-density energy within a very short pulse duration. Such phenomena are called laser-induced plasma ablation. The influence of the laser-induced plasma ablation for tissue is unclear because the ablation mechanism is differing regardless of two lasers provide almost the same power density. The two kinds of lasers' vaporization mechanism (Nanosecond laser output could cause an optical breakdown in the air depending on power density and pulse duration of the laser and Femtosecond laser output could cause a breakdown only on solids surface since pulse irradiation time is shorter than energy transfer time) are evaluated by using thermal damage and destruction of tissue. The experimental results show that nanosecond laser caused vaporization without thermal damage and destruction at the tissue approximant 300 µm away from the ablation area. The pulsed laser which has high power density and longer pulse duration than energy transfer time is suitable for plasma ablation not depending on thermal process.

In this study, the relationship between plasma gas temperature and the bactericidal effects on five of bacteria (Escherichia coli, Pseudomonas aeruginosa, Staphylococcus aureus, Enterococcus faecalis and Bacillus cereus (spore)) in liquid was investigated using a temperature-controllable plasma source. We determined that the bactericidal ability improved as the plasma gas temperature increased. Specifically, the bactericidal ability on E. coli of 80-℃ plasma was enhanced by as much as 6.3 times compared to that of 10-℃ plasma. The relationship between plasma gas temperature and the amount of hydroxyl radical, singlet oxygen, hydrogen peroxide, and ozone introduced into the solution was investigated. Our results also showed that each reactive species production increased by 2.1, 9.0, 1.6, and 17 times, respectively, with 80-℃ compared to 10-℃ plasma. The relationship between the bactericidal ability and amount of reactive species indicated that singlet oxygen and ozone introduced to the solution mostly influenced the bactericidal ability as the plasma gas temperature increased. We conclude that the plasma gas temperature is the crucial parameter for plasma sterilization.

Background Nonthermal atmospheric pressure plasma (NTAPP) has recently received attention as a novel tool in medicine. It is thought that plasma components yield plasma effects such as sterilization, blood coagulation, and wound healing. These effects are produced without thermal damage. We investigated the blood coagulation effect of NTAPP by using a multigas plasma jet. Materials and methods Multigas plasma jets can generate NTAPP by several gas species. In this study, argon, oxygen, helium, nitrogen, mock air, and carbon dioxide were used to generate NTAPP, and blood coagulation times were compared with each plasma-treated sample. The NTAPP blood coagulation effects on whole blood with four different anticoagulants were investigated. In addition, in this study, the effects of plasma treatment on porcine tissues and organs were investigated as in vivo experiment. Results A tendency to coagulate later with argon gas plasma than others was shown. There were no significant differences between oxygen, helium, nitrogen, mock air, and carbon dioxide. Whole blood with each anticoagulant demonstrated fast coagulation by NTAPP treatment. Fast control of the bleeding lesions on porcine stomach and liver by plasma treatment was observed, and no tissue damage due to the plasma treatment was detected by optical microscope. Conclusions These experiments suggest the potential of various gas NTAPPs as a novel medical device to control bleeding lesions.

Background: Indigo naturalis (IND) is an herbal medicine that has been used as an anti-inflammatory agent to treat diseases including dermatitis and inflammatory bowel disease in China. However, the mechanism by which IND exerts its immunomodulatory effect is not well understood. Methods: A murine model of dermatitis and inflammatory bowel disease, both induced by oxazolone (OXA), was treated with IND. The severity of dermatitis was evaluated based on ear thickness measurements and histological scoring. The severity of colitis was evaluated by measuring body weight, histological scoring, and endoscopic scoring. The expression of inflammatory cytokines in ear and colon tissue was evaluated using real-time PCR. 16S rRNA DNA sequencing of feces from OXA-induced colitis mice was performed before and after IND treatment. The effects of IND on OXA-induced colitis were also evaluated after depleting the gut flora with antibiotics to test whether alteration of the gut flora by IND influenced the course of intestinal inflammation in this model. Results: IND treatment ameliorated OXA dermatitis with a reduction in IL-4 and eosinophil recruitment. However, OXA colitis was significantly aggravated in spite of a reduction in intestinal IL-13, a pivotal cytokine in the induction of the colitis. It was found that IND dramatically altered the gut flora and IND no longer exacerbated colitis when colitis was induced after gut flora depletion. Conclusions: Our data suggest that IND could modify the inflammatory immune response in multiple ways, either directly (i.e., modification of the allergic immune cell activity) or indirectly (i.e., alteration of commensal compositions).

An atmospheric pressure plasma source in which the gas temperature can be accurately controlled from below freezing point up to a high temperature has been developed. In general plasma devices, an electrical discharge is passed through a gas at about room temperature to generate plasma, so the plasma is at a temperature higher than room temperature; moreover, the gas temperature is determined by the discharge condition such as discharge power and plasma gas flow rate, so accurate temperature control is difficult. In the plasma source that has been developed in this research, the gas that is to be supplied to the discharge unit is first cooled using a gas cooler and then heated by a heater. The gas temperature of the produced plasma is measured, and feedback is sent to the heater. Thus, plasma at a desired temperature can be generated. Gas temperature control of the plasma over a range from -54 °C to +160 °C with a standard deviation of 1 °C was realized. Spectroscopic characteristics of generated plasma were investigated. This plasma source/technique will realize that effective plasma is applicable for heat-sensitive materials such as paper, textile, polymer, and especially human tissue. Furthermore, it enables us to generate the plasma at optimal gas temperature for chemical reaction of each plasma treatment.

In the medical field, demand for endoscopes has been increasing not only for examination but also for minimally invasive treatment. For endoscopic hemostasis, clips and plasma devices called Argon Plasma Coagulation (APC) are widely used. Although APC has a short hemostasis time, there is a problem with a tissue that is thermally damaged by high plasma temperature. Meanwhile, promotion of blood coagulation by atmospheric low temperature plasma has been reported. Blood points irradiated with the plasma can clot without thermal damage and stop bleeding. Therefore, applying atmospheric low temperature plasma to endoscopic hemostasis can be expected as a minimally invasive hemostatic method. For that purpose, a mini plasma source that can be inserted into a forceps port of endoscope is required. However, conventional plasma source had been manufactured by machining, there was limitation for miniaturization.

In this study, atmospheric nonequilibrium plasmas were generated with six gas species using a multi-gas plasma jet. Singlet oxygen, OH radicals, H radicals, and NO radicals, in reaction with a solution interface, were measured using electron spin resonance. Carbon dioxide plasma generated the largest amount (90 μM) of singlet oxygen at 30 s, and argon-containing vapor gas plasma generated the largest amount (210 μM) of OH radicals. Among the homo-atomic gas species, nitrogen plasma generated the largest amount (130 μM) of OH radicals. In addition, H radicals were generated with argon, helium, and nitrogen plasmas. NO radicals were generated with nitrogen–oxygen plasma, and the largest amount of NO radicals was generated at a 1 : 1 volume ratio. These measurement results of the reactive species generated by individual gas plasmas permit identification of the production processes of reactive species.

Recently, various sterilization methods using atmospheric non-thermal plasma have been studied. Plasma generates reactive species that contributes to the sterilization. Most of the previous studies were reported that water treated by plasma irradiation (plasma treated water) shows sterilization ability. In these experiments, plasma is irradiated to the water surface from above. Therefore, this method is inefficient because the contact area between the plasma and the water is limited. Additionally, a lot of reactive species such as hydroxyl radical (HO·), singlet oxygen (1O 2 ) are not generated by helium or argon plasma.

Our group succeeded in developing a multi-gas plasma jet, which can generate plasma from various gas species. Using this plasma source, reactive species can be investigated in detail, since they can be generated selectively by the supplied gas species. This study aims to investigate the amount of reactive species generated by various gas plasmas, find reactive species that can inactivate bacteria and observe treated bacteria using electron microscope.

Various gas atmospheric nonthermal plasmas were generated using a multigas plasma jet to treat microbial suspensions. Results indicated that carbon dioxide and nitrogen plasma had high sterilization effects. Carbon dioxide plasma, which generated the greatest amount of singlet oxygen than other gas plasmas, killed general bacteria and some fungi. On the other hand, nitrogen plasma, which generated the largest amount of OH radical, killed ≥ 6 log of 11 species of microorganisms, including general bacteria, fungi, acid-fast bacteria, spores, and viruses in 1-15 min. To identify reactive species responsible for bacterial inactivation, antioxidants were added to bacterial suspensions, which revealed that singlet oxygen and OH radicals had greatest inactivation effects.

Atmospheric-pressure plasma jet sources have proven useful for surface treatments; however, conventional plasma and near-plasma sources have limitations in the number of different plasma gas species that they can handle. There has been increasing use of atmospheric plasma for surface treatment and medical applications because the production of atmospheric pressure plasma at low temperatures - from room temperature to around 100°C - has been feasible for a number of years. Processing of materials that are sensitive to temperature, such as living bodies and polymer materials is not easy. In addition, most kinds of plasma processing are chemical reactions with radicals or high reactive species, so that there are suitable temperatures for each process. In this study, to generate plasmas at optimum temperature for each process, a new plasma source in which the plasma gas temperature can be accurately controllable have been developed. In this plasma source, gas supplied from a cylinder is cooled by a gas-cooling device with liquid nitrogen (−196°C). Then the gas is heated to a desired temperature by a heater, and then plasma is generated.

Summary form only given. In recent years, atmospheric non-thermal plasma have been attracted attention in medical field because of its effectiveness for sterilization and wound treatment. However, conventional plasma sources have a limitation in plasma gas species. Plasma can be generated with specific gas such as helium, argon, nitrogen, or air. In our group, the damage-free multi-gas plasma jet source was developed in 2010. This plasma source is free from thermal or electric discharge damage to target materials. Therefore, the plasma treatment is applicable for various materials such as semiconductor, metal, plastic, paper, textile, living body and tissue. And it can generate stable atmospheric plasma jet with helium, argon, oxygen, neon, nitrogen, carbon dioxide, air, and their mixture gas. In surface treatment using the damage-free multi-gas plasma jet, obvious difference of hydrophilization effect by plasma gas species was observed. So, in this study, we examined some biological effects of various gas plasmas. 5 L/min of argon, oxygen, nitrogen, air and oxygen mixed argon plasma were irradiated to E. Coli in agar medium. A clear difference was seen by plasma gas species in the bactericidal effect. Oxygen mixed argon plasma was most effective for sterilization of E. Coli. E. Coli in 90 mm agar medium was sterilized in 10 seconds plasma irradiation. This plasma showed good sterilization effect for other bacteria such as B. subtilis, S. aures, A. niger, and G. stearo. Shape change of bacteria was observed using electron microscope after plasma irradiation. To explore the possibility of plasma irradiation for therapeutic application, its effect on cultured human cell line was investigated. As a result, 90 % of HeLa cells were killed in 40 seconds by irradiation of oxygen mixed argon plasma. The details of the plasma source and the results of these experiments will be presented.

Atmospheric pressure plasma devices provide some advantages in industrial fields because they don't require vacuum equipment. Recently, many types of atmospheric pressure plasma sources have been developed, such as capacitive coupled glow discharge, corona source and plasma jet such as plasma burret. In particular, atmospheric plasma jet sources have been attracted attention because of its usefulness for surface treatment or sterilization. However, conventional plasma jet sources have some limitations in the plasma gas spaces and problem of irradiation damage for target surface such as thermal or electric discharge damage.

In this study, a small-sized plasma jet source with a 3.7 mm head diameter was created via a 3D printer. The jet’s emission properties and OH radical concentrations (generated by argon, helium, and nitrogen plasmas) were investigated using optical emission spectrometry (OES) and electron spin resonance (ESR). As such, for OES, each individual gas plasma propagates emission lines that derive from gases and ambient air inserted into the measurement system. For the case of ESR, a spin adduct of the OH radical is typically observed for all gas plasma treatment scenarios with a 10 s treatment by helium plasma generating the largest amount of OH radicals at 110 μM. Therefore, it was confirmed that a plasma jet source made by a 3D printer can generate stable plasmas using each of the aforementioned three gases.

In this study, non-thermal multi-gas plasma treatments were performed for Tetrodotoxin (TTX) solution, and TTX decomposition was analyzed by liquid chromatography coupled with electrospray time-of-flight mass spectrometry. The TTX mass spectrum signal was reduced by plasma irradiations to different levels by using various gas species. Nitrogen plasma exhibited the optimal capability for TTX decomposition, followed by oxygen, argon, and carbon dioxide plasmas. The TTX concentration decreased 100-fold by nitrogen plasma treatment for 10 min.
