Zusammenfassung Operationsziel Durch die Osteotomie des Ischium‑, Pubis- und Iliumknochens kann das Acetabulum über den Hüftkopf geschwenkt werden, sodass der meist anterolateral vorstehende Anteil des Hüftkopfes wieder überdacht wird. Das Ziel ist der Erhalt des Containments der Hüfte, hiermit ist die Wiederherstellung der Kongruenz zwischen dem lateralisierten Hüftkopf und dem Acetabulum gemeint. Das Acetabulum wirkt so als eine Art Schablone für den Hüftkopf, um eine möglichst sphärische Ausheilung des Hüftkopfes zu erreichen. Indikationen Schwere Morbus-Perthes-Erkrankung mit radiologisch sichtbarer Lateralisation des Hüftkopfes und Head-at-risk-Zeichen. Voraussetzung ist, dass der Hüftkopf sich konzentrisch reponieren lässt (Abduktionsaufnahme oder Arthrographie). Kontraindikationen „Hinged abduction“. Keine konzentrische Reposition des Hüftkopfes möglich. Operationstechnik Arthrographie des Hüftgelenkes zur Bestätigung der Operationsindikation. Darstellung und Osteotomie des Ischiums über einen modifizierten Ludloff-Zugang, Osteotomie des Iliums und Pubis über einen modifizierten Smith-Petersen-Zugang. Verbesserung der Hüftkopfüberdachung durch Schwenken des Acetabulums über den Hüftkopf. Fixation des azetabulären Fragmentes mit Vollgewinde-Kirschner-Drähten oder 3,5-mm-Kortikalisschrauben. Weiterbehandlung Mobilisation an Gehstöcken (kleine Kinder im Rollstuhl). Abstellen des Fußes erlaubt. Teilbelastung für 4 bis 6 Wochen (je nach Alter des Patienten). Danach bei im Röntgen guten Konsolidationszeichen Belastungsaufbau innerhalb 1 bis 2 Wochen. Ergebnisse Sehr gute Ergebnisse hinsichtlich Operationstechnik und Ausheilung in der eigenen Klinik. In einer eigenen noch nicht veröffentlichten Studie mit einem durchschnittlichen Untersuchungszeitraum von 5 Jahren zeigten sich bei 30 Patienten sehr gute klinische und radiologische Ergebnisse nach Tripleosteotomie bei Morbus Perthes.

Zusammenfassung Operationsziel Wiederherstellung der Hüftkongruenz und des Containments durch zentrale Hüftkopfresektion/Reduktion über eine erweiterte chirurgische Hüftluxation unter Erhaltung/Respektierung der Hüftkopfdurchblutung. Eine gleichzeitige oder spätere Reorientierung des Acetabulums durch Triple-Osteotomie oder PAO bei instabiler Hüfte kann in speziellen Fällen notwendig werden. Indikationen Die Indikation zur chirurgischen Hüftkopfreduktion ist praktisch unabhängig vom Alter bei jeglicher inkongruenten, asphärischen Hüftkopfsituation mit „hinged abduction“ (zu erwartende Endsituation wie Stulberg IV und V) gegeben. Dies kann noch bei aktivem wie auch bei bereits ausgeheiltem Morbus Perthes der Fall sein. Zudem kann bei einer Stulberg-V-Situation selbst im adulten Alter noch eine Verbesserung erziel werden. Nach Resektion muss noch ein tragfähiger Resthüftkopf vorhanden bleiben, d. h. mindestens noch 50 % des ausgeweiteten Hüftkopfes. Die dafür beste Planung erfolgt in der „vergleichenden“ 3‑D-Rekonstruktion. Kontraindikationen Völlig zerstörter Knorpel oder Hüftkopf. Operationstechnik Identisches Operationsvorgehen wie für die klassische chirurgische Hüftluxation. Präparation der retinakulären Flaps. Unter Respektierung und in Kenntnis der Gefäßversorgung Spaltung des Hüftkopfes gemäß dem zu entfernenden, nekrotischen Kopfanteil. Bildung eines möglichst sphärischen Hüftkopfes und Verschraubung der beiden Kopfanteile auf Schenkelhalsniveau. Distalisation und Fixierung des großen Trochanters. Je nach Kongruenz und Stabilität des Hüftkopfs in der Hüftpfanne kann eine primäre oder sekundäre Triple-OT oder PAO notwendig werden. Weiterbehandlung Die intraoperative Stabilität des Femurkopfes im Acetabulum muss erzielt worden sein, um eine beckengipsfreie, funktionelle Nachbehandlung zu gewährleisten: Stockentlastung mit Bodenkontakt ist erlaubt; keine aktive Rotation; Flexion aktiv und passiv bis 90 Grad ist erlaubt; vorerst keine spezifische Physiotherapie; je nach Heilungsverlauf sind diese Maßnahmen 8 bis 10 Wochen einzuhalten. Ergebnisse Gemäß unseren publizierten Nachuntersuchungen (aktuell 21 Jahre) sehen wir bei technisch korrekt durchgeführter Operation und korrekter Indikation sowie adäquater Nachbehandlung durchwegs gute Ergebnisse. Nekrosen des reduzierten Hüftkopfs haben wir nie beobachtet. Alle gespaltenen Hüftköpfe, respektive Schenkelhälse sind primär geheilt.

ZusammenfassungFür Chirurgen in der Klinik und Praxis ist der Umgang mit Nahtmaterial das „tägliche Brot“. Daher könnte man annehmen, dass die Kenntnisse dieses Materials kompetent und umfassend sind. Die tägliche Erfahrung zeigt indes ein anderes Bild. Oft sind die Kenntnisse bezüglich Nadelform, Nadelbeschaffenheit sowie Nadelgröße nur marginal und der Chirurg muss sich auf die Kenntnisse seiner OP-Pflegefachkraft verlassen. Die Wahl der Nadel ist für jeden Operationsschritt und jedes Gewebe ausschlaggebend. Die Verwendung des korrekten Nahtmaterials in Bezug auf das zu nähende Gewebe ist jedoch essenziell, einerseits hinsichtlich der Qualität der Naht, aber auch hinsichtlich der Traumatisierung des Gewebes andererseits. Je nach Gewebe sind an Nadel wie auch an Fäden verschiedene Anforderungen zu stellen respektive zu berücksichtigen. Mit diesem Beitrag sollen die wesentlichsten und grundlegendsten Kenntnisse vermittelt werden wie Einfluss der Nadelform und Dimension, Auswirkung von atraumatischen respektive traumatischen (sog. scharfe) Nadeln auf das Gewebe, die Form der Nadelspitze. Das Zusammenspiel zwischen Nadelhalter und Nadel und der Hand des Operateurs bei verschiedenen Anwendungen wird dargestellt. In diesem Beitrag wird das Hauptaugenmerk auf Nadel und Nadelhalter gelegt. In Planung ist ein Nachfolgebeitrag, der sich speziell dem Nahtmaterial widmet. Der Beitrag erhebt nicht den Anspruch auf 100 %ige Vollständigkeit, es sollen jedoch die wesentlichsten, alltäglich vorkommenden Fragen geklärt werden.

Zusammenfassung Operationsziel Die Hüftkopfzirkulation schonende, anatomische Reposition und sichere Stabilisierung von Schenkelhalsfrakturen im Kindesalter über einen transglutealen Zugang. Indikationen Intra-extraartikuläre proximale Femurfrakturen (Schenkelhalsfrakturen) AO 31-M/2.1 I‑III; 31-M/3.1 I‑III; 31-M/3.2 I‑III. Kontraindikationen Keine. Operationstechnik Präparation eines Muskellappens durch Ablösen des proximalen M. vastus lateralis inklusive des anterolateralen Anteils des M. glutaeus medius vom proximalen Femur respektive Trochanter major. Ablösen des glutaeus minimus von der Gelenkkapsel und Weghalten nach dorsal, ohne die Insertion des Muskels vollständig abzulösen. Die anterolaterale Gelenkkapsel kann nun vollständig exponiert werden. Arthrotomie der Gelenkkapsel und Darstellen des Schenkelhalses. Unter Sicht nun vorsichtige, kontrollierte Reposition der Fraktur unter Schutz der retinakulären Gefäße. Weiterbehandlung Mobilisation an Gehstöcken. Abstellen des Fußes erlaubt. Zur vollständigen Anheilung der Hüftabduktoren sollte eine aktive Abduktion sowie passive Adduktion für 4 bis 6 Wochen (je nach Alter des Patienten) vermieden werden. Ergebnisse In der eigenen Klink zeigten sich in den letzten 10 Jahren exzellente Ergebnisse bei 29 Patienten nach Behandlung von kindlichen Schenkelhalsfrakturen mit diesem Operationszugang. Eine operationsbedingte Femurkopfnekrose trat nicht auf.

Zusammenfassung Operationsziel Sichere und stabile Fixierung metaphysärer sowie epiphysärer Frakturen (Salter-Harris I–IV) mittels korrekter Bohrdraht(Kirschner[K]-Draht)-Osteosynthese, in der Folge als K‑Draht-Osteosynthese (OS) bezeichnet. Indikationen Gemäß der AO(Arbeitsgemeinschaft für Osteosynthesefragen)-Kinderklassifikation der langen Röhrenknochen (AO Pediatric Comprehensive Classification of Long-Bone Fractures [PCCF]) alle Salter-Harris- und metaphysären Frakturen sowie Frakturen des Fuß- und Handskelettes, unabhängig von der Repositionsart, geschlossen oder offen, sofern eine Adaptationsosteosynthese eine hinreichende Stabilität zulässt. Eine K‑Draht-OS erfordert immer eine zusätzliche Fixierung/Ruhigstellung in einem Gipsverband. Kontraindikationen Alle diaphysären Frakturen, sofern ein K‑Draht nicht im Sinne der Markraumschienung verwendet wird. Nicht korrekt reponierte respektive nicht reponierbare Frakturen. Operationstechnik Nach geschlossener oder offener, möglichst anatomischer Reposition werden unter Durchleuchtungskontrolle 1, 2, gelegentlich 3 K-Drähte pro Fragment eingebracht. Wichtig ist dabei, dass die K‑Drähte das zu fixierende Fragment sowie das Hauptfragment (Metaphyse) optimal fassen. Es muss daher möglich sein, mit dem Durchleuchtungsgerät eine streng seitliche sowie korrekte anteroposteriore Aufnahme machen zu können. Dabei ist darauf zu achten, dass man das Gerät in die entsprechende Ebene schwenken kann. Ein Drehen der Extremität sollte auf ein Minimum beschränkt werden. Durch eine zusätzliche Manipulation zwecks Durchleuchtung könnten die zuvor optimal reponierten Fragmente erneut dislozieren. Dies wiederum kann zu einer schlechten K‑Draht-Fixierung führen. Je nach Morphologie der Fraktur, Größe der Fragmente und Lokalisation der Fraktur (Humerus, Unterarm, Femur oder Tibia, Hand oder Fuß) muss die K‑Draht-Technik angepasst werden. Diese kann sein: monolateral gekreuzt, monolateral divergierend auf- oder absteigend oder die häufigste angewendete aufsteigend gekreuzte Technik. Die K‑Drähte werden in der Regel über Hautniveau belassen und umgebogen. Somit können sie ohne erneute Narkose in der Ambulanz entfernt werden. Man muss sich bewusst sein, dass der K‑Draht weder eine Kompressions- noch eine Neutralisations-OS ist, sondern immer nur eine Adaptation. Daher braucht eine K‑Draht-OS immer eine zusätzliche Ruhigstellung mittels Gips oder konfektionierter Schiene. Weiterbehandlung Ruhigstellung im Gipsverband für 4 bis 5 Wochen abhängig vom Alter. Ergebnisse Bei technisch optimal durchgeführter Fixation und korrekter Indikation für eine K‑Draht-OS sowie adäquater Nachbehandlung sind die Ergebnisse sehr gut bis gut.

Aims We aimed to evaluate the health-related quality of life (HRQoL) in children with supracondylar humeral fractures (SCHFs), who were treated following the recommendations of the Paediatric Comprehensive AO Classification, and to assess if HRQoL was associated with AO fracture classification, or fixation with a lateral external fixator compared with closed reduction and percutaneous pinning (CRPP). Methods We were able to follow-up on 775 patients (395 girls, 380 boys) who sustained a SCHF from 2004 to 2017. Patients completed questionnaires including the Quick Disabilities of the Arm, Shoulder and Hand questionnaire (QuickDASH; primary outcome), and the Pediatric Quality of Life Inventory (PedsQL). Results An AO type I SCHF was most frequent (327 children; type II: 143; type III: 150; type IV: 155 children). All children with type I fractures were treated nonoperatively. Two children with a type II fracture, 136 with a type III fracture, and 141 children with a type IV fracture underwent CRPP. In the remaining 27 children with type III or IV fractures, a lateral external fixator was necessary for closed reduction. There were no open reductions. After a mean follow-up of 6.3 years (SD 3.7), patients with a type I fracture had a mean QuickDASH of 2.0 (SD 5.2), at a scale of 0 to 100, with lower values representing better HRQoL (type II: 2.8 (SD 10.7); type III: 3.3 (SD 8.0); type IV: 1.8 (SD 4.6)). The mean function score of the PedsQL ranged from 97.4 (SD 8.0) for type I to 96.1 (SD 9.1) for type III fractures, at a scale of 0 to 100, with higher values representing better HRQoL. Conclusion In this cohort of 775 patients in whom nonoperative treatment was chosen for AO type I and II fractures and CRPP or a lateral external fixator was used in AO type III and IV fractures, there was equally excellent mid- and long-term HRQoL when assessed by the QuickDASH and PedsQL. These results indicate that the treatment protocol followed in this study is unambiguous, avoids open reductions, and is associated with excellent treatment outcomes. Prerequisite is possible concentric reduction of the femoral head (confirmed by preoperative abduction radiograph or arthrography).Hinged abduction. Impossible concentric reduction of the femoral head.Hip arthrography to confirm the indication of the triple pelvic osteotomy is recommended. Osteotomy of the ischial bone by a modified Ludloff approach. Osteotomy of pubic and iliac bone by anterior approach (Smith Peterson/bikini incision). Restoration of hip congruence and containment through central femoral head resection/reduction via an extended surgical hip dislocation, while maintaining/respecting the femoral head blood flow. Simultaneous or subsequent reorientation of the acetabulum by triple TPO (Pediatric Triple Osteotomy) or PAO (Peri-Acetabular Osteotomy) may be necessary.Surgical hip dislocation with femoral head reduction can be performed at any age in cases with hinge abduction and Stulberg class IV and V deformity. Procedure indicated for patients with active or healed disease. After the resection, a viable residual femoral head must remain, i.e. at least 50% of the expanded femoral head, which is best planned using "comparative" 3D reconstruction.Completely destroyed cartilage or femoral head.The same surgical procedure as described for classic surgical hip dislocation is followed. Preparation of retinacular flaps. With detailed knowledge of the vascular supply and precise execution of this technique, blood supply to the femoral head will be preserved; once safely surgically dislocated, the femoral head and neck can be split and the necrotic part of the femoral head removed. Reformation of the femoral head as spherical as possible is achieved by screw fixation of the femoral neck to align the two articular parts of the femoral head. Distalization and fixation of the great trochanter helps to restore offset (functional femoral neck length). Depending on the congruence and stability of the femoral head in the acetabulum, a primary TPO or PAO may also be necessary.Intraoperative stability must be achieved to ensure functional posttreatment without a hip spica cast. Walking with crutches with toe contact only is advised. Active rotation is not allowed. Active and passive flexion up to 90° allowed. These measures have to be observed for 8-10 weeks. Then, active physiotherapy rehabilitation may commence, depending on healing, as assessed clinically and radiologically.Our published follow-up examinations (currently 21 years) show consistently good results with a technically correct operation and correct indication as well as adequate follow-up treatment. Hintergrund Die elastisch-stabile intramedulläre Nagelung (ESIN) hat sich in den letzten 2 Jahrzenten weltweit zum Goldstandard in der Versorgung der Frakturen im Kindesalter entwickelt. Werden die biomechanischen und technischen Vorgaben eingehalten, handelt es sich um ein sicheres und effizientes Verfahren. Problematik Das Alter des Kindes, der Markraumdurchmesser in Relation zur Knochendicke und hohes Körpergewicht können limitierende Faktoren der ESIN sein. In gleicher Weise beeinflusst auch die Komplexität der Fraktur in Kombination mit technischen Mängeln die Qualität der Versorgung. Die einfache Anwendung der ESIN verleitet zu deren unsorgfältigem Einsatz und v. a. zur Vernachlässigung der unabdingbaren Einhaltung der biomechanischen Prinzipien. Wesentliche Probleme aufseiten des Chirurgen sind die ungenügende Analyse der Fraktur, die daraus resultierende relativ falsche Indikationsstellung, die Nichteinhaltung der technischen Vorgaben, wie z. B. nicht korrekte Wahl der Implantatdicke sowie Eintrittsstellen, keine innere Aufspannung/kein 3-Punkte-Kontakt, das Ineinanderverwinden der Nägel, das sog. Korkenzieherphänomen, Nagelperforationen am proximalen Femurende und der Ausbruch von zusätzlichen Knochenkeilen. Optimale Versorgung des Jugendlichen Um diese sicherzustellen, muss eine alters-, gewichts- und knochenmorphologieadaptierte Methode gewählt werden, womit die Fraktur sicher, stabil und definitiv fixiert werden kann. Die Versorgung soll kind- und jugendgerecht und damit möglichst atraumatisch erfolgen. Es dürfen keine Fehlstellungen toleriert werden, da kein ausreichendes Korrekturpotenzial mehr vorhanden ist. Die Osteosynthese sollte belastbar sein. Resümee Die ESIN sollte nur bei Kindern mit noch offenen Fugen angewandt werden. Durch den Einsatz von „end caps“ kann ihre axiale Belastung um mehrere Faktoren erhöht werden. Bei ungenügender Erfahrung mit der ESIN und/oder problematischen Gegebenheiten des Patienten und/oder der Fraktur ist die Plattenosteosynthese eine der ersten Alternativen, um eine sichere und suffiziente Frakturstabilisierung zu erreichen., Background Elastic stable intramedullary nailing (ESIN) has developed into the gold standard of treatment for femoral fractures in childhood worldwide. It is a safe and efficient procedure if the biomechanical and technical prerequisites are adhered to. Objectives The age of the child, the diameter of the bone marrow cavity in relation to bone thickness and high body weight can be limiting factors of ESIN. In the same manner the quality of treatment is influenced by the complexity of the fracture in combination with technical shortcomings. The simple application of ESIN can induce an inattentive approach and in particular neglecting the indispensable adherence to biomechanical principles. The major problems on the part of surgeons are an insufficient analysis of the fracture, the resulting relatively false indications and non-compliance to technical prerequisites. This could be due to incorrect selection of implant thickness and entry site, lack of internal clamping and 3-point contact, corkscrewing of nails, nail perforation at the proximal end of the femur and fragmentation of additional bone wedges. Optimal treatment of adolescents In order to fulfill these conditions, a method adapted to age, weight and bone morphology must be selected with which the fracture can be securely, stably and definitively fixed. The treatment should be carried out in a manner which is suitable for children and adolescents and should, therefore, be as atraumatic as possible. Malpositioning cannot be tolerated because sufficient correction potential is no longer present. The osteosynthesis should be capable of withstanding pressure. Conclusion The ESIN procedure should only be used in children who still have open epiphyseal plates. The axial load can be increased several-fold by the use of end caps. In cases of insufficient experience with ESIN, problematic circumstances of the patient and/or the fracture, plate osteosynthesis is one of the foremost alternatives in order to achieve a secure and sufficient stabilization of the fracture.

For surgeons in the clinic and practice, the handling of suture material is the "daily bread". Therefore, one might assume that knowledge of this material is competent and comprehensive. However, daily experience shows a different picture. Often, the knowledge regarding needle shape, needle composition as well as needle size is only marginal and the surgeon has to rely on the knowledge of his OR nurse.The choice of needle is critical for each surgical step and each tissue; however, the use of the correct suture material in relation to the tissue to be sutured is essential, not only with respect to the quality of the suture but also with respect to the traumatization of the tissue. Depending on the tissue, different requirements must be placed on or taken into account for both the needle and the sutures. The purpose of this article is to provide the most essential and basic knowledge, such as the influence of needle shape and dimensions, effect of atraumatic or traumatic (so called sharp) needles on the tissue and shape of the needle tip. The interaction between the needle holder and the needle and the surgeon's hand in different applications is presented. In this present article, the main focus is on the needle and needle holder. A follow-up article specifically dedicated to sutures is being planned. The article does not claim to be 100% complete but the most essential, everyday questions should be clarified.Für Chirurgen in der Klinik und Praxis ist der Umgang mit Nahtmaterial das „tägliche Brot“. Daher könnte man annehmen, dass die Kenntnisse dieses Materials kompetent und umfassend sind. Die tägliche Erfahrung zeigt indes ein anderes Bild. Oft sind die Kenntnisse bezüglich Nadelform, Nadelbeschaffenheit sowie Nadelgröße nur marginal und der Chirurg muss sich auf die Kenntnisse seiner OP-Pflegefachkraft verlassen. Die Wahl der Nadel ist für jeden Operationsschritt und jedes Gewebe ausschlaggebend. Die Verwendung des korrekten Nahtmaterials in Bezug auf das zu nähende Gewebe ist jedoch essenziell, einerseits hinsichtlich der Qualität der Naht, aber auch hinsichtlich der Traumatisierung des Gewebes andererseits. Je nach Gewebe sind an Nadel wie auch an Fäden verschiedene Anforderungen zu stellen respektive zu berücksichtigen. Mit diesem Beitrag sollen die wesentlichsten und grundlegendsten Kenntnisse vermittelt werden wie Einfluss der Nadelform und Dimension, Auswirkung von atraumatischen respektive traumatischen (sog. scharfe) Nadeln auf das Gewebe, die Form der Nadelspitze. Das Zusammenspiel zwischen Nadelhalter und Nadel und der Hand des Operateurs bei verschiedenen Anwendungen wird dargestellt. In diesem Beitrag wird das Hauptaugenmerk auf Nadel und Nadelhalter gelegt. In Planung ist ein Nachfolgebeitrag, der sich speziell dem Nahtmaterial widmet. Der Beitrag erhebt nicht den Anspruch auf 100 %ige Vollständigkeit, es sollen jedoch die wesentlichsten, alltäglich vorkommenden Fragen geklärt werden.

Transgluteal approach for anatomical reduction of femoral neck fractures (extra-intraarticular) in children under preservation of the blood supply of the femoral head.Femoral neck fractures AO 31-M/2.1 I‑III; 31-M/3.1 I‑III; 31-M/3.2 I‑III.None.Preparation of a muscular flap including the proximal insertion of the vastus lateralis muscle and approximately one third of the gluteus medius muscle. Exposure of the anterolateral hip capsule and capsulotomy followed by controlled reduction of the fracture fragments without compromising the retinacular vessels.Touch-down weightbearing for 4-6 weeks (age dependent). To protect the healing of the abductors, active abduction or passive adduction prohibited for 4-6 weeks. Consolidation radiographs 4-6 weeks postoperatively.Excellent results in 29 patients subsequently treated in the last 10 years by the transgluteal approach. Ablösen des glutaeus minimus von der Gelenkkapsel und Weghalten nach dorsal, ohne die Insertion des Muskels vollständig abzulösen. Die anterolaterale Gelenkkapsel kann nun vollständig exponiert werden. Arthrotomie der Gelenkkapsel und Darstellen des Schenkelhalses. Unter Sicht nun vorsichtige, kontrollierte Reposition der Fraktur unter Schutz der retinakulären Gefäße.Mobilisation an Gehstöcken. Abstellen des Fußes erlaubt. Zur vollständigen Anheilung der Hüftabduktoren sollte eine aktive Abduktion sowie passive Adduktion für 4 bis 6 Wochen (je nach Alter des Patienten) vermieden werden.In der eigenen Klink zeigten sich in den letzten 10 Jahren exzellente Ergebnisse bei 29 Patienten nach Behandlung von kindlichen Schenkelhalsfrakturen mit diesem Operationszugang. Eine operationsbedingte Femurkopfnekrose trat nicht auf.

Fur Chirurgen mit weniger Erfahrung in der Behandlung von Kinderfrakturen stellt die suprakondylare Humerusfraktur eine besondere Herausforderung dar. Die hauptsachliche Ursache liegt neben der geringen Erfahrung vor allem in einer mangelhaften Diagnostik im Sinne schlechter Rontgenuntersuchungen und der damit verbundenen schwierigen Beurteilung und der daraus folgenden ungenugenden Behandlung. Es sollen die haufigsten Diagnostik- und Beurteilungsfehler sowie Fehleinschatzungen dargelegt und diskutiert werden. Mit Tipps und Tricks werden Hinweise zur optimalen Versorgung dieser Fraktur gegeben.

Zur Daumenrekonstruktion nach traumatischer Amputation hat sich auch die Kallusdistraktion mittels Distraktionsfixateur etabliert. Die Wahl des geeigneten Verfahrens hangt von der Amputationshohe, dem Alter, Beruf und vom funktionellen Anspruch des Patienten ab. Wir berichten uber unsere Erfahrungen bei 5 Patienten, welche nach traumatischen Daumenamputationen auf Hohe der Basis der Grundphalanx mit einem in unserer Klinik in Zusammenarbeit mit einer spezialisierten Firma (Rotomed AG, Allmenstrasse 4, CH-4512 Bellach, www.rotomed.ch ) entwickelten semi-zirkularen Ringfixateur System behandelt wurden. Bei allen Patienten konnte eine zufriedenstellende Verlangerung von durchschnittlich 26,2 mm erreicht werden. Es traten keine Achsenabweichung oder revisionsbedurftigen Komplikationen auf. Der Hand Michigan Outcome Score betrug durchschnittlich 82,2%.

Background and purpose — To achieve a common understanding when dealing with long bone fractures in children, the AO Pediatric Comprehensive Classification of Long Bone Fractures (AO PCCF) was introduced in 2007. As part of its final validation, we present the most relevant fracture patterns in the lower extremities of a representative population of children classified according to the PCCF. Patients and methods — We included patients up to the age of 17 who were diagnosed with 1 or more long bone fractures between January 2009 and December 2011 at either of 2 tertiary care university hospitals in Switzerland. Patient charts were retrospectively reviewed. Results — More lower extremity fractures occurred in boys (62%, n = 341). Of 548 fractured long bones in the lower extremity, 25% involved the femur and 75% the lower leg. The older the patients, the more combined fractures of the tibia and fibula were sustained (adolescents: 50%, 61 of 123). Salter-Harris (SH) fracture patterns represented 66% of single epiphyseal fractures (83 of 126). Overall, 74 of the 83 SH patterns occurred in the distal epiphysis. Of all the metaphyseal fractures, 74 of 79 were classified as incomplete or complete. Complete oblique spiral fractures accounted for 57% of diaphyseal fractures (120 of 211). Of all fractures, 7% (40 of 548) were classified in the category "other", including 29 fractures that were identified as toddler’s fractures. 5 combined lower leg fractures were reported in the proximal metaphysis, 40 in the diaphysis, 26 in the distal metaphysis, and 8 in the distal epiphysis. Interpretation — The PCCF allows classification of lower extremity fracture patterns in the clinical setting. Re-introduction of a specific code for toddler’s fractures in the PCCF should be considered.

Cubitus varus deformity is a well-known late complication of supracondylar fractures in children. In this retrospective study, the primary objective was to compare clinical and radiologic outcomes of lateral closing-wedge osteotomy with either internal fixation or external fixation in pediatric patients with cubitus varus deformities.From 2010 to 2017, 35 consecutive patients with cubitus varus deformities secondary to supracondylar fractures were included in this study. After corrective osteotomy was performed via a limited lateral approach, the method of definitive fixation was chosen between internal and external. Retrospectively, patients who underwent external fixation on the lateral aspect of the elbow were defined as group I (n = 16) whereas patients with unilateral single-plate fixation were defined as group II (n = 19). The functional outcome was evaluated using the Mayo Elbow Performance Score and Flynn criteria.No significant difference in age was found between the 2 groups (P = .15). Significantly lower costs, a shorter operation duration, smaller scars, and a shorter time for plaster cast use postoperatively were found in group I (P.001). No nonunion or failure of fixation was found. No significant difference was noted in postoperative elbow range of motion or Mayo Elbow Performance Score (P = .64). Both groups achieved satisfactory functional and cosmetic results.In pediatric patients with cubitus varus, both methods of fixation after lateral closing-wedge corrective osteotomy are reliable, with a low rate of complications and satisfactory functional results. External fixation is more advantageous in terms of easier preoperative planning, shorter operative times, lower costs, and easier postoperative fixation removal.

Operationsziel Sichere und stabile Fixierung metaphysärer sowie epiphysärer Frakturen (Salter-Harris I–IV) mittels korrekter Bohrdraht(Kirschner[K]-Draht)-Osteosynthese, in der Folge als K‑Draht-Osteosynthese (OS) bezeichnet. Indikationen Gemäß der AO(Arbeitsgemeinschaft für Osteosynthesefragen)-Kinderklassifikation der langen Röhrenknochen (AO Pediatric Comprehensive Classification of Long-Bone Fractures [PCCF]) alle Salter-Harris- und metaphysären Frakturen sowie Frakturen des Fuß- und Handskelettes, unabhängig von der Repositionsart, geschlossen oder offen, sofern eine Adaptationsosteosynthese eine hinreichende Stabilität zulässt. Eine K‑Draht-OS erfordert immer eine zusätzliche Fixierung/Ruhigstellung in einem Gipsverband. Kontraindikationen Alle diaphysären Frakturen, sofern ein K‑Draht nicht im Sinne der Markraumschienung verwendet wird. Nicht korrekt reponierte respektive nicht reponierbare Frakturen. Operationstechnik Nach geschlossener oder offener, möglichst anatomischer Reposition werden unter Durchleuchtungskontrolle 1, 2, gelegentlich 3 K-Drähte pro Fragment eingebracht. Wichtig ist dabei, dass die K‑Drähte das zu fixierende Fragment sowie das Hauptfragment (Metaphyse) optimal fassen. Es muss daher möglich sein, mit dem Durchleuchtungsgerät eine streng seitliche sowie korrekte anteroposteriore Aufnahme machen zu können. Dabei ist darauf zu achten, dass man das Gerät in die entsprechende Ebene schwenken kann. Ein Drehen der Extremität sollte auf ein Minimum beschränkt werden. Durch eine zusätzliche Manipulation zwecks Durchleuchtung könnten die zuvor optimal reponierten Fragmente erneut dislozieren. Dies wiederum kann zu einer schlechten K‑Draht-Fixierung führen. Je nach Morphologie der Fraktur, Größe der Fragmente und Lokalisation der Fraktur (Humerus, Unterarm, Femur oder Tibia, Hand oder Fuß) muss die K‑Draht-Technik angepasst werden. Diese kann sein: monolateral gekreuzt, monolateral divergierend auf- oder absteigend oder die häufigste angewendete aufsteigend gekreuzte Technik. Die K‑Drähte werden in der Regel über Hautniveau belassen und umgebogen. Somit können sie ohne erneute Narkose in der Ambulanz entfernt werden. Man muss sich bewusst sein, dass der K‑Draht weder eine Kompressions- noch eine Neutralisations-OS ist, sondern immer nur eine Adaptation. Daher braucht eine K‑Draht-OS immer eine zusätzliche Ruhigstellung mittels Gips oder konfektionierter Schiene. Weiterbehandlung Ruhigstellung im Gipsverband für 4 bis 5 Wochen abhängig vom Alter. Ergebnisse Bei technisch optimal durchgeführter Fixation und korrekter Indikation für eine K‑Draht-OS sowie adäquater Nachbehandlung sind die Ergebnisse sehr gut bis gut.

Zusammenfassung : Operationsziel: Geschlossene, anatomische Reposition und sichere Fixation von problematischen suprakondylären Typ-III- und Typ-IV-Humerusfrakturen, die mit den herkömmlichen Operationsmethoden nur schwierig geschlossen zu behandeln sind. Indikationen: Gemäß der AO-Kinderklassifikation der suprakondylären Humerusfrakturen vom TypIII und IV: Frakturen, welche nicht geschlossen mittels üblicher Repositionsmethoden reponierbar sind sowie Frakturen, die nicht mittels der üblichen, gekreuzten perkutanen Kirschner-Draht-Technik zu fixieren sind. Bei schweren Schwellungszuständen, offener Fraktur oder initial neurologischen und/oder vaskulären Problemen ("pulseless pink hand") sowie bei mehrfachverletzten Kindern, welche eine optimale Rehabilitation benötigen und die Extremität gipsfrei sein sollte. Bei Kindern mit Komorbiditäten (z.B. Anfälle, Spastizität), die eine bessere Stabilität benötigen. Kontraindikationen: Prinzipiell keine Kontraindikationen Operationstechnik: Im nichtreponierten Zustand unter Durchleuchtungskontrolle Einbringen einer einzelnen Schanz-Schraube in den lateralen (radialen) Aspekt des distalen Fragments, welches sich in der streng seitlichen Röntgenprojektion als "Sand-Uhr"- bzw. Kreisform des Capitulum humeri darstellt. Je nach Größe dieses distalen Fragments kann die Schanz-Schraube rein epiphysär oder metaphysär liegen. Danach in absolut streng seitlicher Projektion des distalen Humerus im Bereich des meta-diaphysären Übergangs Einbohren einer 2. Schanz-Schraube unabhängig von der Ersten, die möglichst rechtwinklig zur Längsachse des Humerus in der a.-p.-Ebene zu liegen kommen sollte, um spätere Manipulationen mittels "Joy-Stick"-Technik zu erleichtern. Sind die beiden Schanz-Schrauben mehr oder weniger in beiden Ebenen parallel, so ist die Fraktur praktisch anatomisch reponiert. Nach erreichter Reposition Feinjustierung aller Achskomponenten. Sicherung der Flexion/Extension mittels einem von radial, distal eingebrachten sog. Anti-Rotations-Kirschner-Drahts, der die Stabilität signifikant erhöht und eine Drehung des distalen Fragments um die einzelne Schanz-Schraube verhindert. Postoperative Behandlung: Keine zusätzliche Gipsruhigstellung notwendig. Es sollte eine funktionelle Nachbehandlung erfolgen. Ergebnisse: Gemäß unserer Langzeitstudien bewegen die meisten Kinder bereits zum Zeitpunkt der ambulanten Pin-Entfernung in der Frakturambulanz ihren Ellbogen weitgehend normal. Bei einer Follow-up-Zeit über 40Monate hatten 30/31Kindern eine seitengleiche Achse und Beweglichkeit.

Korrektur einer posttraumatischen oder anlagebedingten Rotationsfehlstellung des Unterschenkels in Relation zur Gegenseite mittels Osteotomie und Stabilisation uber winkelstabile Platten. Posttraumatischer Rotationsfehler des Unterschenkels nach fehlverheilter Unterschenkelfraktur. Vermehrte Innendrehung der Tibia bei rebellischem Klumpfus, sofern die Fuskomponenten korrekt behandelt wurden. Idiopathische, angeborene ubermasige Innen- bzw. Ausenrotation, sofern das Kind kosmetisch relevant beeintrachtigt ist (z. B. Hanseln durch andere Kinder), durch die Fehlstellung subjektiv Probleme hat oder der eindeutige Wunsch des Kindes/Eltern vorhanden ist (relative Indikation). Rotationsfehler des Beins, der durch Huftmalrotation bedingt ist. Bei Kindern unter 3 Jahren sollte die normale Entwicklung der Unterschenkeltorsion durch das Wachstum abgewartet werden. Supramalleolar durchgefuhrte Korrekturosteotomie und Fixierung mittels einer padiatrischen 90°-3,5-mm-LCP-Kondylenplatte. Osteotomie im Ubergang vom mittleren zum distalen Tibiadrittel und Fixierung mit gerader 2,7-mm-/3,5-mm-LCP-4-Loch-Platte. Bei der supramalleolaren Osteotomie kann gipsfrei und unter schmerzadaptierter Belastung weiterbehandelt werden. Bei der Osteotomie im distalen Tibiadrittel ist fur 4–5 Wochen eine Ruhigstellung im Unterschenkelgips mit Teilbelastung notwendig. In beiden Fallen erfolgt die Entfernung des Osteosynthesematerials 1 Jahr postoperativ. Je nach Alter des Kindes ist eine adaquate knocherne Konsolidation innerhalb von 4–6 Wochen zu erwarten. Bei standardisierter Operationstechnik unter Schonung der distalen tibialen Wachstumsfuge sind im Verlauf keine Achsabweichungen durch Epiphysenfugenverschluss oder Korrekturverlust zu erwarten.

Zusammenfassung : Operationsziel : Definitive Reposition des chronisch luxierten Radiuskopfs zur Verhinderung einer möglichen späteren Schädigung oder Funktionseinbuße. Indikationen : Bei frischen Verletzungen immer. Bei chronischer Luxation bei Beschwerden. Kontraindikationen : Prinzipiell keine. Operationstechnik : Die Operationstechnik hat sich prinzipiell nach der zu korrigierenden Fehlstellung oder Grundproblematik sowie dem zur Verfügung stehenden Instrumentarium und Implantat zu richten. Es gibt mehrere Techniken: alleinige angulierende Ulnaosteotomie (kurzes Intervall nach Unfall < 3 Monate), angulierende und verlängernde Ulnaosteotomie (meist > 3 Monate nach Unfall); ein zusätzliches offenes Vorgehen ist selten notwendig bzw. sollte unterlassen werden. Postoperative Behandlung : Abhängig vom Alter des Kinds und der gewählten Technik (Fixateur externe, Platte). Es sollte aber nach Möglichkeit immer eine gipsverbandfreie Nachbehandlung angestrebt werden (bessere Funktion). Ergebnisse : Die Ergebnisse hängen von der Erfahrung des Operateurs, der Genauigkeit der Planung und der gewählten Technik ab. Eine intensive physiotherapeutische Nachbehandlung ist unumgänglich, häufig initial unter Einsatz einer Motorschiene. Korrekturverluste und Rezidive sind bekannt und nicht selten. Diese kommen bei ungenügender Stabilisierung oder Zentrierung zustande; aussagekräftige Zahlen gibt es nicht, doch die klinische Erfahrung bestätigt dies nur allzu oft

Background Backyard trampolines are immensely popular among children, but are associated with an increase of trampoline-related injuries. The aim of this study was to evaluate radiographs of children with trampoline related injuries and to determine the risk factors. Methods Between 2003 and 2009, 286 children under the age of 16 with backyard trampoline injuries were included in the study. The number of injuries increased from 13 patients in 2003 to 86 in 2009. The median age of the 286 patients was 7 years (range: 1–15 years). Totally 140 (49%) patients were males, and 146 (51%) females. Medical records and all available diagnostic imaging were reviewed. A questionnaire was sent to the parents to evaluate the circumstances of each injury, the type of trampoline, the protection equipment and the experience of the children using the trampoline. The study was approved by the Institutional Ethics Committee of the University Hospital of Bern. Results The questionnaires and radiographs of the 104 patients were available for evaluation. A fracture was sustained in 51 of the 104 patients. More than 75% of all patients sustaining injuries and in 90% of patients with fractures were jumping on the trampoline with other children at the time of the accident. The most common fractures were supracondylar humeral fractures (29%) and forearm fractures (25%). Fractures of the proximal tibia occurred especially in younger children between 2–5 years of age. Conclusions Children younger than 5 years old are at risk for specific proximal tibia fractures (“Trampoline Fracture”). A child jumping simultaneously with other children has a higher risk of suffering from a fracture.

Zusammenfassung: Operationsziel: Sichere und suffiziente proximale Femurosteotomie mit niedriger Komplikationsrate durch exakte und zuverlässige präoperative Planung. Indikationen: Congenitale Schenkelhalspseudarthrose. Posttraumatische Schenkelhalspseudarthrose. Coxa vara. Kontraindikationen: Keine. Bei bestimmten Pathologien wie z. B. der Hüftdysplasie sind alleinige Korrekturen des proximalen Femurs gerade bei älteren Kindern häufig nicht ausreichend und sollten mit einer Beckenosteotomie kombiniert werden. Operationstechnik: Exakte präoperative Planung der gewünschten Korrektur. Lateraler Zugang zum proximalen Femur. Platzieren eines Anteversionsdrahts auf den ventralen Schenkelhals. Setzen des Positions-Kirschner-Drahts. Einbringen der 2,8-mm-Kirschner-Drähte in den Schenkelhals. Osteotomieren des proximalen Femurs nach Markierung der Rotation mit Kirschner-Drähten oder der oszillierenden Säge. Schieben der Platte über die Kirschner-Drähte und Fixation der Platte an das proximale Fragment durch Austauschen der Kirschner-Drähte mit winkelstabilen Schrauben. Reposition des Femurschafts an die Platte und Fixation mit winkelstabilen oder Kortikalisschrauben. Readaptation des Muskels über der Platte. Weiterbehandlung: Je nach verwendeter Platte ist keine äußere Fixation notwendig. Mobilisation an Gehstöcken mit Teilbelastung je nach Alter für 4-6Wochen. Ergebnisse: In einer eigenen Publikation suffiziente Korrektur und stabile Osteosynthese bei 22Patienten (30Hüften). Niedrige Komplikationsraten. Die exzellenten Ergebnisse wurden auch durch weitere Studien unterstützt.

Traumatic amputation of the thumb at the proximal phalanx or the metacarpophalangeal joint can be treated by distraction lengthening depending on the location of the amputation, the patient's age, occupation and functional demands. We report the results of proximal phalanx lengthening in 5 patients using a semicircular ring-type external fixator device. This prototype was developed at our clinic in collaboration with a specialised small company (Rotomed AG, Allmenstrasse 4, CH-4512 Bellach, www.rotomed.ch) based on the principles of Ilizarov's external fixator. In all patients, subjective and objective results were satisfactory (26.2 mm distraction length and an average Michigan Hand Outcome Score of 82.2%) without any rotational/axial deformities or complications requiring revision surgery.

Clavicle reconstruction is a rare operation. In most cases a mid-shaft defect of the clavicle is bridged by using different grafting techniques or musculo-osteous flaps. In some clinical situations where reconstruction is not a suitable option claviculectomy as a salvation procedure has proven to be an acceptable solution. In the paediatric population the challenge of both the cosmetic and the functional result attempting reconstruction of large bone defects is of higher demand. To our knowledge, this is the first case of a successful clavicle reconstruction with a sufficient follow-up using a free vascularised fibula graft in a child. This case provides a technique description, considerations in the paediatric population, an overview of other techniques used, and a long-term follow-up.

Legg-Calve-Perthes disease commonly leads to substantial deformity of the femoral head and proximal femur. A nonspherical femoral head can lead to a secondary distortion of the acetabulum and premature osteoarthritis of the hip. Several surgical treatment options are available to improve the morphology of the proximal femoral epiphysis and thus the biomechanics of the hip. Advancing the greater trochanter addresses some of the extra-articular impingement. Depending on the quality of cartilage of the aspheric femoral head, the intra-articular deformity can be helped with intertrochanteric osteotomies, either valgus or varus, to bring the good cartilage into the weight-bearing area of the acetabulum. Unfortunately, these osteotomies produce secondary deformities such as a shortened femoral neck with further weakening of the abductor complex. This article describes new surgical techniques that create relative lengthening of the femoral neck and a reduction of the size and improvement in the sphericity of an excessively large and extruded femoral head. These techniques are intended to restore the congruence of the affected hip joint.

Traditionally arthrotomy has rarely been performed during surgery for slipped capital femoral epiphysis (SCFE). As a result, most pathophysiological information about the articular surfaces was derived clinically and radiographically. Novel insights regarding deformity-induced damage and epiphyseal perfusion became available with surgical hip dislocation.We (1) determined the influence of chronicity of prodromal symptoms and severity of SCFE deformity on severity of cartilage damage. (2) In surgically confirmed disconnected epiphyses, we determined the influence of injury and time to surgery on epiphyseal perfusion; and (3) the frequency of new bone at the posterior neck potentially reducing perfusion during epimetaphyseal reduction.We reviewed 116 patients with 119 SCFE and available records treated between 1996 and 2011. Acetabular cartilage damage was graded as +/++/+++ in 109 of the 119 hips. Epiphyseal perfusion was determined with laser-Doppler flowmetry at capsulotomy and after reduction. Information about bone at the posterior neck was retrieved from operative reports.Ninety-seven of 109 hips (89%) had documented cartilage damage; severity was not associated with higher slip angle or chronicity; disconnected epiphyses had less damage. Temporary or definitive cessation of perfusion in disconnected epiphyses increased with time to surgery; posterior bone resection improved the perfusion. In one necrosis, the retinaculum was ruptured; two were in the group with the longest time interval. Posterior bone formation is frequent in disconnected epiphyses, even without prodromal periods.Addressing the cause of cartilage damage (cam impingement) should become an integral part of SCFE surgery. Early surgery for disconnected epiphyses appears to reduce the risk of necrosis. Slip reduction without resection of posterior bone apposition may jeopardize epiphyseal perfusion.Level IV, retrospective case series. The modified Dunn procedure has the potential to restore the anatomy in hips with slipped capital femoral epiphyses (SCFE) while protecting the blood supply to the femoral head and minimizing secondary impingement deformities. However, there is controversy about the risks associated with the procedure and mid- to long-term data on clinical outcomes, reoperations, and complications are sparse.Among patients treated with a modified Dunn procedure for SCFE, we report on (1) hip pain and function as measured by the Merle d'Aubigné and Postel score, Drehmann sign, anterior impingement test, limp, and ROM; (2) the cumulative survivorship at minimum 10-year followup with endpoints of osteoarthritis (OA) progression (at least one Tönnis grade), subsequent THA, or a Merle d'Aubigné and Postel score15; (3) radiographic anatomy of the proximal femur measured by slip angle, α angle, Klein line, and sphericity index; and (4) the risk of subsequent surgery and complications.Between 1998 and 2005, all patients who presented to our institution with SCFE were treated with a modified Dunn procedure; this approach was applied regardless of whether the slips were mild or severe, acute or chronic, and all were considered potentially eligible here. Of the 43 patients (43 hips) thus treated during that time, 42 (98%) were available for a minimum 10-year followup (mean, 12 years; range, 10-17 years) and complete radiographic and clinical followup was available on 38 hips (88%). The mean age of the patients was 13 years (range, 9-18 years). Ten hips (23%) presented with a mild, 27 hips (63%) with a moderate, and six hips (14%) with a severe slip angle. Pain and function were measured using the Merle d'Aubigné and Postel score, limp, ROM, and the presence of a positive anterior impingement test or Drehmann sign. Cumulative survivorship was calculated according to the method of Kaplan-Meier with three defined endpoints: (1) progression by at least one grade of OA according to Tönnis; (2) subsequent THA; or (3) a Merle d'Aubigné and Postel score15. Radiographic anatomy was assessed with the slip angle, Klein line, α angle, and sphericity index.The Merle d'Aubigné and Postel score improved at the latest followup from 13 ± 2 (7-14) to 17 ± 1 (14-18; p0.001), the prevalence of limp decreased from 47% (18 of 38 hips) to 0% (none in 38 hips; p0.001), the prevalence of a positive Drehmann sign decreased from 50% (nine of 18 hips) to 0% (none in 38 hips; p0.001), and both flexion and internal rotation improved meaningfully. Cumulative survivorship was 93% at 10 years (95% confidence interval, 85%-100%). Radiographic anatomy improved, but secondary impingement deformities remained in some patients, and secondary surgical procedures included nine hips (21%) with screw removal and six hips (14%) undergoing open procedures for impingement deformities. Complications occurred in four hips (9%) and no hips demonstrated avascular necrosis on plain radiographs.In this series, the modified Dunn procedure largely corrected slip deformities with little apparent risk of progression to avascular necrosis or THA and high hip scores at 10 years. However, secondary impingement deformities persisted in some hips and of those some underwent further surgical corrections.Level IV, therapeutic study.

Aims Several studies have reported the safety and efficacy of subcapital re-alignment for patients with slipped capital femoral epiphysis (SCFE) using surgical dislocation of the hip and an extended retinacular flap. Instability of the hip and dislocation as a consequence of this surgery has only recently gained attention. We discuss this problem with some illustrative cases. Materials and Methods We explored the literature on the possible pathophysiological causes and surgical steps associated with the risk of post-operative instability and articular damage. In addition, we describe supplementary steps that could be used to avoid these problems. Results The causes of instability may be divided into three main groups: the first includes causes directly related to SCFE (acetabular labral damage, severe abrasion of the acetabular cartilage, flattening of the acetabular roof and a bell-shaped deformity of the epiphysis); the second, causes not related to the SCFE (acetabular orientation and poor quality of the soft tissues); the third, causes directly related to the surgery (capsulotomy, division of the ligamentum teres, shortening of the femoral neck, pelvi-trochanteric impingement, previous proximal femoral osteotomy and post-operative positioning of the leg). Conclusion We present examples drawn from our clinical practice, as well as possible ways of reducing the risks of these complications, and of correcting them if they happen. Osteotomies of the proximal femur and stable fixation of displaced femoral neck fractures are demanding operations. An LCP Paediatric Hip Plate was developed to make these operations safer and less demanding. The article focuses on the surgical technique and critically analyses the device.Between 2006 and 2008, 30 hips in 22 patients underwent surgery. Patients' demographics, perioperative details, postoperative outcome and complications were retrospectively collected and analysed.Patients' diagnoses included persistent congenital hip dysplasia (n = 4), neuropathic hip dysplasia (n = 9), idiopathic ante/retroversion (n = 8), femoral neck fracture (n = 3), Perthes' disease (n = 2), deformity after slipped capital femoral epiphysis (SCFE), congenital femoral neck pseudarthrosis, deformity after pelvic tumour resection and malunion following proximal femoral fracture (one each). In 21 of 22 patients, the postoperative radiographs showed corrections as planned. Two cases had to be revised for screw loosening. Intraoperative handling using the plate was excellent in all cases.In our case series of 30 hip operations, the LCP Paediatric Hip Plate was shown to be safe and applicable in the clinical setting with excellent results and a low complication rate. We consider that the LCP Paediatric Hip Plate is a valuable device for correction of pathological conditions of the proximal femur and for fixation of displaced femoral neck fractures in children. Larger studies should be carried out to better quantify the risk of clinically relevant complications.

Purpose A common treatment for pediatric femur fractures is intramedullary nail (IMN) insertion. Elastic stable intramedullary nails (ESINs) are often used for these procedures in heavier patients, but the potential for complications and malunion is greater. We describe here a rigid IMN specifically designed for adolescents, the adolescent lateral entry femoral nail (ALFN). The purpose of this study was to compare the recovery and complications for patients treated with ESINs to those treated with the ALFN. Methods Our study design was a retrospective cohort study. We performed a review of medical records of 22 children ages 10–17 requiring surgical fixation of a femur fracture for a 2½-year period. Patients selected for the study had traumatic diaphyseal femur fractures and were treated with ESINs without end-caps or ALFNs. Our analyses evaluated injury, surgical, and outcome information for all patients. Results Twenty-two patients were eligible for inclusion and were divided into two groups according to their treatment: the ESIN group with 7 patients and the ALFN group with 15 patients. We then performed a comparison of complications and recovery for these patients. The mean time to full weight-bearing was significantly less for the ALFN group (4.1 weeks; SD, 2.2), than the ESIN group (9.4 weeks; SD 3.9). There was no statistical difference in the incidence of major or minor complications. Conclusions Older, heavier pediatric patients treated for femur fracture with ALFNs had a shorter recovery time than similar patients treated with ESINs. However, the outcomes for both groups were satisfactory.

Surgical procedures with use of traditional techniques to reposition the proximal femoral epiphysis in the treatment of slipped capital femoral epiphysis are associated with a high rate of femoral head osteonecrosis. Therefore, most surgeons advocate in situ fixation of the slipped epiphysis with acceptance of any persistent deformity in the proximal part of the femur. This residual deformity can lead to secondary osteoarthritis resulting from femoroacetabular cam impingement.We retrospectively assessed the cases of twenty-three patients with slipped capital femoral epiphysis after surgical correction with a modified Dunn procedure, an approach that included surgical hip dislocation. The study reviewed the clinical status and radiographs made at the time of surgery, as well as the intraoperative findings. At a minimum follow-up of twenty-four months after surgery, the motion of the treated hip was compared with the motion of the contralateral hip, and the radiographic findings related to the anatomy of the femoral head-neck junction, as well as signs of early osteoarthritis or osteonecrosis, were evaluated.Twenty-one patients had excellent clinical and radiographic outcomes with respect to hip function and radiographic parameters. Two patients who developed severe osteoarthritis and osteonecrosis had a poor outcome. The mean slip angle of the femoral head of 47.6° preoperatively was corrected to a normal value of 4.6° (p0.0001). The mean flexion and internal rotation postoperatively were 107.3° and 37.8°, respectively. The mean range of motion of the treated hips was not significantly different (p0.05) from that of the normal, contralateral hips. Of the eight hips that were considered unstable in the intraoperative clinical assessment, six had been considered stable preoperatively.The treatment of slipped capital femoral epiphysis with the modified Dunn procedure allows the restoration of more normal proximal femoral anatomy by complete correction of the slip angle, such that probability of secondary osteoarthritis and femoroacetabular cam impingement may be minimized. The complication rate from this procedure in our series was low, even in the treatment of unstable slipped capital femoral epiphysis, compared with alternative procedures described in the literature for fixation of slipped capital femoral epiphysis.

BACKGROUND: Long-term results after partial, extended, or complete resection of lateral discoid meniscus in children revealed knee arthritis. The purpose of this study was to evaluate whether the operative approach, arthrotomy or arthroscopy, has an impact on the outcome and the development of arthritis. METHODS: A retrospective comparison of 2 well matching groups totaling 40 children with symptomatic lateral discoid meniscus (48 knees, mean age 8.9 years, 13 male and 27 female patients). Meniscus resection was performed via mini-arthrotomy in group 1 (n=17 patients, 20 knees) and arthroscopically in group 2 (n=23 patients, 28 knees). RESULTS: In the follow-up (mean 57 months in group 1, 62 months in group 2), functional results indicated a trend to better results in the International Knee Documentation Committee score (P=0.12) and in the Lysholm score for group 1 (P=0.13) but not in the Ikeuchi score (P=0.48). The comparison of the radiographic arthritis grading in the follow-up showed no significant arthritis in either group (P=0.22). The overall complication rate was similar in both groups (2/20, 10% in group 1; 3/28, 12% in group 2). CONCLUSIONS: Most likely because of the appropriate visualization of the children's joint and the easier instrumentation, the mini-arthrotomy led to slightly superior results compared with those after arthroscopic resection regarding functional outcome and 5 years after surgery. We can recommend the mini-arthrotomy for the resection of lateral discoid meniscus particularly in young children with narrow joint spaces and for surgeons that are not familiar with arthroscopies of small joints. Indikationen : Richten sich am Humerus nach zwei Gesichtspunkten: nach Morphologie (stark verschobene Querfrakturen, lange Schräg- und Spiralbrüche sowie komplexe Brüche) und Stabilität (völlig instabile Frakturen) sowie heute nach sozioökonomischen Gründen und Komfort. Daher kennt man absolute und relative Indikationen. Kontraindikationen : Stabile, unverschobene Brüche. Operationstechnik : Richtet sich nach den Prinzipien der ESIN-Technik (elastisch-stabile Markraumschienung). Klare Darstellung der Eintrittspunkte auf der lateralen Seite; Eröffnung des Mark raums mit dem Pfriem oder Bohrer; Einbringen der beiden Nägel, wobei ein Nagel C-förmig, der andere S-förmig vorgebogen ist. Weiterbehandlung : Gipsverbandfreie, funktionelle Weiterbehandlung; je nach Angst und Schmerz kann eine Schlinge verwendet werden. Ergebnisse : Bei korrekter Indikation und Technik ist am Humerus immer ein perfektes Ergebnis zu fordern; Fehler liegen immer beim Chirurgen!

BACKGROUND: Percutaneous Kirschner wire fixation represents the classic treatment for displaced supracondylar humeral fractures in childhood. This type of treatment first requires satisfactory reduction of the fracture. Failure to achieve a satisfactory reduction or inadequate stabilization can result in instability of the fracture fragments, which can result in either an unsatisfactory cosmetic or functional outcome. In our experience, these problems can be overcome with the use of a small lateral external fixator. METHODS: Between 1999 and 2005, thirty-one of 170 Gartland type-III supracondylar humeral fractures were treated with a lateral external fixator. The outcome of treatment was analyzed with regard to limb alignment, elbow movement, cosmetic appearance, and patient satisfaction. RESULTS: In twenty-eight of the thirty-one patients, a satisfactory reduction was achieved with closed methods. All children except one had a normal or good range of movement. The cosmetic result was excellent in all cases. All of the children and their parents stated that they would choose this treatment again. CONCLUSIONS: The use of a small lateral external fixator seems to be a safe alternative for the treatment of displaced supracondylar fractures of the humerus when a closed reduction appears to be unattainable by means of manipulation alone or when sufficient stability is not achieved with standard methods of Kirschner wire fixation.

The purpose of this new classification compendium is to republish the Orthopaedic Trauma Association's (OTA) classification. The OTA classification was originally published in a compendium of the Journal of Orthopaedic Trauma in 1996. It adopted The Comprehensive Classification of the Long Bones developed by Müller and colleagues and classified the remaining bones. In this compendium, the introductory chapter reviews new scientific information about classifying fractures that has been published in the last 11 years. The classification is presented in a revised format that is easier to follow. The OTA and AO classification will now have a unified alpha-numeric code eliminating the differences that have existed between the 2 codes. The code was significantly revised for the clavicle and scapula, foot and hand, and patella. Dislocations have been expanded on an anatomic basis and for most joints will be coded separately. This publication should stimulate new developments and interest in a unified language to code and classify fractures. Further improvements in classification will result in better patient care and clinical research.

The AO Pediatric Expert Group and the AO Pediatric Classification Group, in cooperation with the AO Investigation and Documentation Group introduce and present the first comprehensive classification of pediatric long bone fractures. The anatomy is related to the 4 long bones and their 3 segments defined as proximal (1), shaft (2) and distal (3). It is further described by the fracture subsegment recorded as epiphyseal (E), metaphyseal (M) and diaphyseal (D), whereby proximal and distal fractures are classified as E or M and shaft fractures are always D. The distinction between metaphyseal and diaphyseal fractures is achieved by localizing the center of fracture lines with regard to a square drawn over the respective growth plates. The morphology of the fracture is documented by a subsegment-specific child pattern code, a severity code as well as an additional code for displacement of specific fractures such as supracondylar fractures and radial heads. The classification process requires trained observers to read standard radiographic images.
