Self-management education and continuous support are needed to improve patients’ confidence to take such active role. One way to do this is through eHealth technologies. However, those technologies can only be successful when actively used in daily practice and when integrated in overall care. Therefore, this study investigated how a self-management eHealth technology could be implemented that emphasises the active role of patients in their care.Methods: The service modelling method was utilized as implementation strategy. The design process consisted of five phases with salient stakeholders and consortium members of a European project to develop the service model. Studies with salient stakeholders were carried out in three different countries (Italy, Estonia, the Netherlands). A combination between face-to-face and online methods facilitated the participatory design process.Results: Due to the pan-European context, different stakeholders in the three countries were identified. Research nurses and case managers were not yet established in practice but once implemented, expected to contribute to optimal implementation. During service modelling, a crucial step was revealed: providing self-management training before technology use to let patient familiarise with the concept of taking an active role. As HCPs felt that they were not necessarily equipped to guide patients in terms of self-management, they also should have access to such self-management training.Conclusion: By demonstrating a way for implementation while emphasising patients’ active role, we also showed the complexity of the method in two ways. First, by demonstrating the fine line between the descriptive and prescriptive model. Thus, showcasing the need to recognize that prescriptive models may be hampered by the delay in changing work practices. Second, by highlighting the importance of identifying country-specific differences in the pan-European context, revealing that service modelling is not a one-size-fits-all approach.Keywords: service model, self-management, patient empowerment, responsibility, eHealth, COPD

2. Hybrid direct drive with a two-sided ultraviolet laser.

Abstract

This paper presents a "hybrid" approach to direct drive inertial confinement fusion that can exploit a high-energy gas laser with two opposed beams. The target and driver are asymmetric, much like experiments performed on the National Ignition Facility, but have been designed to benefit from scale and their particular compatibility with a fusion power plant. The imploded masses (and areal densities) are increased by a factor of 12 (3) relative to findings by Abu-Shawareb et al. [Phys. Rev. Lett. 129, 075001 (2022)] and provide a path to high-gain implosions that robustly ignite. The design also mitigates common concerns such as laser imprint and cross-beam energy transfer. We discuss the rationales for a hybrid target, the methods used to control implosion symmetry, and the implication(s) for inertial fusion energy. [ABSTRACT FROM AUTHOR]

Raman and Brillouin amplification are two schemes for amplifying and compressing short laser pulses in plasma. Analytical models have already been derived for both schemes, but the full consequences of these models are little known or used. Abstract

Absorption covers the physical processes which convert intense photon flux into energetic particles when a high-power laser illuminates optically-thick matter. It underpins important petawatt-scale applications today, e.g., medical-quality proton beam production. However, development of ultra-high-field applications has been hindered since no study so far has described absorption throughout the entire transition from the classical to the quantum electrodynamical (QED) regime of plasma physics. Here we present a model of absorption that holds over an unprecedented six orders-of-magnitude in optical intensity and lays the groundwork for QED applications of laser-driven particle beams. We demonstrate 58% efficient \gamma-ray production at $1.8\times 10^{25}~\mathrm{W~ cm^{-2}}$ and the creation of an anti-matter source achieving $4\times 10^{24}\ \mathrm{positrons}\ \mathrm{cm^{-3}}$, $10^{6}~\times$ denser than of any known photonic scheme. These results will find applications in scaled laboratory probes of black hole and pulsar winds, \gamma-ray radiography for materials science and homeland security, and fundamental nuclear physics., Comment: 10 pages, 5 figures

Electronic health (eHealth) might play an important role in adherence to interventions for the self-management of COPD and CHF symptoms by facilitating and supporting home-based care.Methods: In this pilot study, an eHealth self-management intervention was developed based on paper versions of multi-morbid exacerbation action plans and evaluated in patients with both COPD and CHF. Self-reporting of increased symptoms in diaries was linked to an automated decision support system that generated self-management actions, which was communicated via an eHealth application on a tablet. After participating in self-management training sessions, patients used the intervention for a maximum of four months. Adherence to daily symptom diary completion and follow-up of actions were analyzed. An add-on sensorized (Respiro®) inhaler was used to analyze inhaled medication adherence and inhalation technique.Results: In total, 1148 (91%) of the daily diaries were completed on the same day by 11 participating patients (mean age 66.8 ± 2.9 years; moderate (55%) to severe (45%) COPD; 46% midrange left ventricular function (LVF) and 27% reduced LVF). Seven patients received a total of 24 advised actions because of increased symptoms of which 11 (46%) were followed-up. Of the 13 (54%) unperformed advised actions, six were “call the case manager”. Adherence to inhaled medication was 98.4%, but 51.9% of inhalations were performed incorrectly, with “inhaling too shortly” (< 1.25 s) being the most frequent error (79.6%).Discussion: Whereas adherence to completing daily diaries was high, advised actions were inadequately followed-up, particularly the action “call the case manager”. Inhaled medication adherence was high, but inhalations were poorly performed. 9. Kinematic Constraints on Absorption of Ultraintense Laser Light

Abstract

We derive upper and lower bounds on the absorption of ultraintense laser light by solids as a function of fundamental laser and plasma parameters. These limits emerge naturally from constrained optimization techniques applied to a generalization of the laser-solid interaction as a strongly-driven, relativistic, two degree of freedom Maxwell-Vlasov system. We demonstrate that the extrema and the phase-space-averaged absorption must always increase with intensity, and increase most rapidly when $10^{18} < I_L \ \lambda_L^2 < 10^{20}$ W $\mu$m$^2/$cm$^{2}$. Our results indicate that the fundamental empirical trend towards increasing fractional absorption with irradiance therefore reflects the underlying phase space constraints.

Abstract

Transport modeling of idealized, cone-guided fast ignition targets indicates the severe challenge posed by fast-electron source divergence. The hybrid particle-in-cell [PIC] code Zuma is run in tandem with the radiation-hydrodynamics code Hydra to model fast-electron propagation, fuel heating, and thermonuclear burn. The fast electron source is based on a 3D explicit-PIC laser-plasma simulation with the PSC code. This shows a quasi two-temperature energy spectrum, and a divergent angle spectrum (average velocity-space polar angle of 52 degrees). Transport simulations with the PIC-based divergence do not ignite for > 1 MJ of fast-electron energy, for a modest 70 micron standoff distance from fast-electron injection to the dense fuel. However, artificially collimating the source gives an ignition energy of 132 kJ. To mitigate the divergence, we consider imposed axial magnetic fields. Uniform fields ~50 MG are sufficient to recover the artificially collimated ignition energy. Experiments at the Omega laser facility have generated fields of this magnitude by imploding a capsule in seed fields of 50-100 kG. Such imploded fields are however more compressed in the transport region than in the laser absorption region. When fast electrons encounter increasing field strength, magnetic mirroring can reflect a substantial fraction of them and reduce coupling to the fuel. Abstract

Simulations of ignition-scale fast ignition targets have been performed with the new integrated Zuma-Hydra PIC-hydrodynamic capability. We consider an idealized spherical DT fuel assembly with a carbon cone, and an artificially-collimated fast electron source. We study the role of E and B fields and the fast electron energy spectrum. For mono-energetic 1.5 MeV fast electrons, without E and B fields, the energy needed for ignition is E_f^{ig} = 30 kJ. This is about 3.5x the minimal deposited ignition energy of 8.7 kJ for our fuel density of 450 g/cm^3. Including E and B fields with the resistive Ohm's law E = \eta J_b gives E_f^{ig} = 20 kJ, while using the full Ohm's law gives E_f^{ig} > 40 kJ. This is due to magnetic self-guiding in the former case, and \nabla n \times \nabla T magnetic fields in the latter. Using a realistic, quasi two-temperature energy spectrum derived from PIC laser-plasma simulations increases E_f^{ig} to (102, 81, 162) kJ for (no E/B, E = \eta J_b, full Ohm's law). This stems from the electrons being too energetic to fully stop in the optimal hot spot depth., Comment: Minor revisions in response to referee comments

The program consisted of four modules: 1) activity coach for ambulant activity monitoring and real-time coaching of daily activity behavior, 2) web-based exercise program for home exercising, 3) self-management of COPD exacerbations via a triage diary on the web portal, including self-treatment of exacerbations, and 4) teleconsultation. Twenty-nine COPD patients were randomly assigned to either the intervention group (telehealth program for 9 months) or the control group (usual care). Page hits on the web portal showed the use of the program, and the Client Satisfaction Questionnaire showed satisfaction with received care. The telehealth program with decision support showed good satisfaction (mean 26.4, maximum score 32). The program was accessed on 86% of the treatment days, especially the diary. Patient adherence with the exercise scheme was low (21%). Health care providers seem to play an important role in patients' adherence to telehealth in usual care. Future research should focus on full-scale implementation in daily care and investigating technological advances, like gaming, to increase adherence. Abstract

Implementing information and communications technology (ICT) at scale requires evaluation processes to capture the impact on users as well as the infrastructure into which it is being introduced. For older adults living with cognitive impairment, this requires evaluation that can accommodate different levels of cognitive impairment, alongside input from family and formal caregivers, plus stakeholder organisations. The European Horizon 2020 project INdependent LIving support Functions for the Elderly (IN LIFE) set out to integrate 17 technologies into a single digital platform for older people living with cogni- tive impairment plus their families, care providers and stakeholders. The IN LIFE evaluation took place across six national pilot sites to examine a number of variables including impact on the users, user acceptance of the individual services and the overall platform, plus the economic case for the IN LIFE platform. The results confirmed the interest and need among older adults, family caregivers, formal caregivers and stakeholders, for information and communications technology (ICT). Relative to the baseline, quality of life improved and cognition stabilised; however, there was an overall reluctance to pay for the platform. The findings provide insights into existing barriers and challenges for adoption of ICT for older people living with cognitive impairment.

Abstract

For more than half a century, researchers around the world have been engaged in attempts to achieve fusion ignition as a proof of principle of various fusion concepts. Following the Lawson criterion, an ignited plasma is one where the fusion heating power is high enough to overcome all the physical processes that cool the fusion plasma, creating a positive thermodynamic feedback loop with rapidly increasing temperature. In inertially confined fusion, ignition is a state where the fusion plasma can begin "burn propagation" into surrounding cold fuel, enabling the possibility of high energy gain. While "scientific breakeven" (i.e., unity target gain) has not yet been achieved (here target gain is 0.72, 1.37 MJ of fusion for 1.92 MJ of laser energy), this Letter reports the first controlled fusion experiment, using laser indirect drive, on the National Ignition Facility to produce capsule gain (here 5.8) and reach ignition by nine different formulations of the Lawson criterion.

Abstract

Aim To describe the methods and feasibility of the nutritional intervention carried out within the SPRINTT Randomized cotrolled trial. We also illustrate how nutrition interventionists identified participants at risk of malnutrition and the lessons learnt from the nutrition intervention. Findings SPRINTT nutrition intervention was well-received by the majority of the participants. It was mainly carried out using tailored nutrition counselling, but also other means of delivering the intervention were successfully used. Compared with a standard nutrition prescription, an individualized protocol to diagnose malnutrition and follow-up by tailored nutrition counselling helped achieve nutritional targets more effectively in spite of diversity of population in nutritional habits and in some cases reluctance to accept changes. Message The SPRINTT nutrition intervention was feasible and allowed flexibility to the varying needs and cultural differences of this heterogeneous population of frail, older Europeans. It may serve as a model to educate and improve nutrition among community-dwelling older people at risk of mobility limitations. Supplementary Information The online version contains supplementary material available at 10.1007/s41999-020-00438-4., Background The “Sarcopenia and Physical Frailty in Older People: Multicomponent Treatment Strategies” (SPRINTT) project sponsored a multi-center randomized controlled trial (RCT) with the objective to determine the effect of physical activity and nutrition intervention for prevention of mobility disability in community-dwelling frail older Europeans. We describe here the design and feasibility of the SPRINTT nutrition intervention, including techniques used by nutrition interventionists to identify those at risk of malnutrition and to carry out the nutrition intervention. Methods SPRINTT RCT recruited older adults (≥ 70 years) from 11 European countries. Eligible participants (n = 1517) had functional limitations measured with Short Physical Performance Battery (SPPB score 3–9) and low muscle mass as determined by DXA scans, but were able to walk 400 m without assistance within 15 min. Participants were followed up for up to 3 years. The nutrition intervention was carried out mainly by individual nutrition counseling. Nutrition goals included achieving a daily protein intake of 1.0–1.2 g/kg body weight, energy intake of 25–30 kcal/kg of body weight/day, and serum vitamin D concentration ≥ 75 mmol/L. Survey on the method strategies and feasibility of the nutrition intervention was sent to all nutrition interventionists of the 16 SPRINTT study sites. Results Nutrition interventionists from all study sites responded to the survey. All responders found that the SPRINTT nutrition intervention was feasible for the target population, and it was well received by the majority. The identification of participants at nutritional risk was accomplished by combining information from interviews, questionnaires, clinical and laboratory data. Although the nutrition intervention was mainly carried out using individual nutritional counselling, other assisting methods were used as appropriate. Conclusion The SPRINTT nutrition intervention was feasible and able to adapt flexibly to varying needs of this heterogeneous population. The procedures adopted to identify older adults at risk of malnutrition and to design the appropriate intervention may serve as a model to deliver nutrition intervention for community-dwelling older people with mobility limitations. Abstract

An investigation of the interaction between chlorpromazine (CPZ) and monolayers of 1,2-dipalmitoyl-sn-3-glycerophosphatidylcholine (DPPC) and 1,2-dipalmitoyl-sn-3-glycero[phospho-rac-(1-glycerol)] (DPPG), both at the air/water interface and in transferred Langmuir-Blodgett films is presented. The IR and compressibility results demonstrate that the effect from CPZ could not be correlated with temperature changes in the subphase for pure monolayers, in contrast to models proposed by other authors.
