Amaç: Tiroid nodülleri, toplumda sık görülen ve görüntüleme yöntemleri arttıkça insidantal saptanma oranları da gittikçe artan bir sağlık problemidir. Bu nedenle ki tiroid nodüllerine klinik yaklaşımda takip edilecek ve operasyona yönlendirilecek olguların ayrımı için, sitopatolojik Bethesda sınıflaması çok önemli bir yer tutmaktadır. Bu sınıflama sistemi 6 kategoriden oluşmaktadır ve tiroid nodüllerinin hepsini bir sınıflamaya koymak mümkündür. Biz çalışmamıza tiroid ince iğne aspirasyon sitoloji (İİAS) yapılan ve sonrasında cerrahi opere edilen olguları retrospektif olarak inceleyerek, kendi merkezimizdeki uyumu değerledirmeyi amaçladık. Materyal ve Metot: Çalışmamıza, S.B.Ü Diyarbakır Gazi Yaşargil Eğitim ve Araştırma Hastanesinde, 2014-2019 yılları arasında opera edilen ve öncesinde tiroid İİAB uygulanan 425 olgu dâhil ettik. Tiroid Bethesda sınıf 4 (Foliküler neoplazi/Foliküler neoplazi kuşkusu) olgularının histopatolojik tanıları ile papiller mikrokarsinom ve Hashimoto/lenfositik tiroidit tanısı alan vakaların sitopatolojik tanıları ayrıca belirtilmiştir. Bulgular: Bethesda tiroid sitopatoloji sınıflamasına göre, olguların 53’ü yetersiz, 193’ü benign, 85’i önemi belirsi atipi/önemi belirsiz foliküler lezyon, 22’si foliküler neoplazi açısından kuşkulu, 61’i malignite açısından kuşkulu ve 11’i malign sitoloji olarak raporlanmıştır. Histopatolojik olarak tiroid papiller mikrokarsinom tanısı alan toplam 33 olgunun çoğunluğu 10 (%31) önemi belirsiz atipi tanısı almış olup sırasıyla; yetersiz 7 (%21), benign 7 (%21), malignite açısından kuşkulu 7 1(%21), foliküler neoplazi kuşkusu 1 (%3) ve malign sitoloji 1 (%3) tanıları almıştır. Hashimoto tiroiditi ya da lenfositik tiroidit tanısı alan toplam 51 vakanın sitoloji preparatında aldığı tanılar sırasıyla; benign 17 (%33), önemi belirsiz atipi 16 (%51), malignite kuşkulu 11 (%21), yetersiz 4 (%8), foliküler neoplazi kuşkusu 2 (%4) ve malign sitoloji 1 (%2)’dir.Sonuç: Çalışmamızda, vaka oranlarımız genel olarak literatür ile uyumludur. Ancak papiller mikrokarsinomların yanlış negatif tanı oranını arttırabileceğini, Hashimoto tiroiditi/lenfositik tiroidit olgularının ise önemli belirsiz atipi ve malignite kuşkulu sitoloji tanılarını arttırabileceği gözlemlenmiştir. Tek merkezle yaptığımı çalışmamızda sitolojik tanı veririken olgunun bu yönlerden klinik, sonografik ve laboratuar bulgularıyla birlikte değerlendirilmesi gerektiğini böylece bu durumu aşabileceğimizi düşünmekteyiz., Aim: Thyroid nodules are a common health problem, which is common in the community, and its rates of detection are increasing in proportion to the increasing use of imaging methods. For this reason, cytopathological classification of Bethesda has a very important place for the differentiation of the cases that will be followed in the clinical approach to the thyroid nodules and directed to the operation. This classification system consists of 6 categories and it is possible to put all thyroid nodules into one classification. We aimed to evaluate the compliance in our center by retrospectively examining the patients who underwent thyroid fine needle aspiration cytology (FNAC) and subsequently operated surgically.Material and Method: In our study, we included 425 patients who were operated between 2014-2019 and who had undergone thyroid FNAC in the Diyarbakır Gazi Yaşargil Training and Research Hospital. Histopathological diagnoses of thyroid Bethesda class 4 (Follicular neoplasia / Follicular neoplasia suspicion) cases and cytopathological diagnoses of cases diagnosed with papillary microcarcinoma and Hashimoto / lymphocytic thyroiditis were also evaluated.Results: According to the Bethesda thyroid cytopathology classification, 53 of the cases were reported as nondiagnostic, 193 as benign, 85 as Atypia of Undetermined Significance or Follicular Lesion of Undetermined Significance (AUS / FLUS), 22 as suspicious in terms of follicular neoplasia, 61 as suspicious in terms of malignancy, and 11 as malignant cytology. 33 cases diagnosed with thyroid papillary microcarcinoma, cytopathologically; 10 (31%) were reported as AUS / FLUS, 7 (%21) were non-diagnostic, 7 (%21) were benign, 7 (%21) were Suspicious for Malignancy, 1 (%3) were Follicular Neoplasm or Suspicious for a Follicular Neoplasm, and 1 (%3) was reported as malignant cytology. The diagnoses of 51 cases diagnosed with Hashimoto thyroiditis or lymphocytic thyroiditis in the cytology preparation are; benign 17 (33%), AUS/FLUS 16 (51%), suspicious of malignancy 11 (21%), nondiagnostic 4 (8%), Follicular Neoplasm or Suspicious for a Follicular Neoplasm 2 (4%) and malignant cytology 1 (2%).Conclusion: In our study, our case rates are generally compatible with the literature. However, we observed that papillary microcarcinomas may increase the false negative diagnosis rate, while Hashimoto's thyroiditis /Lymphocytic thyroiditis cases may increase cases of AUS/FLUS and suspicious of malignancy cytology. In our study with a single center, we think that the case should be evaluated together with clinical, sonographic and laboratory findings in these aspects, and that we can overcome this situation.

Psödohipoparatiroidizm (PHPT); hedef organın parathormona (PTH) yanıt vermediği kalıtımsal bir bozukluktur. Biyokimyasal olarak; hipokalsemi, hiperfosfatemi ve PTH yüksekliği ile karakterizedir. PTH uygulamasına verilen yanıt belirgin derecede düşüktür. Tip 1A, biyokimyasal özelliklere ek olarak Albright herediter osteodistrofisi (AHO) olarak bilinen karakteristik somatik bir fenotipe de sahiptir. Bu fenotipin, kısa boy, yuvarlak yüz, frontal bombelik, brakidaktili, obezite özelikleri bulunmaktadır. Burada biz, hipokalsemi ve Albright herediter osteodistrofisi tanısı koyduğumuz; kalsiyum, aktif D vitamini ile tedavi ettiğimiz olgumuzu sunduk. Pseudohypoparathyroidism (PHPT); is a hereditary disorder in which the target organ does not respond to parathormone (PTH). Biochemically; it is characterized by the hypocalcemia, hyperphosphatemia, and elevated PTH. The response to PTH administration is markedly lower. In addition to it's biochemical properties, Type 1A also has a characteristic somatic phenotype known as Albright hereditary osteodystrophy (AHO). This phenotype is characterized by short stature, round face, frontal bubble, brachydactyly, obesity. Here, we present a patient, diagnosed with Albright’s hereditary osteodystrophy and hypocalcemia that is treated by calcium, vitamin D .

46 XX malesexual disorder is a very rare disease. SRY (sex-determining region Y) locatedin the Y chromosome is a main sex-specific gen location. It is thought that thetranslocation of Y chromosome part containing this gene region to the Xchromosome is responsible for the disorder. A 35 year-old male patient wasadmitted to the outpatient clinic with complaints of infertility and decreasedsexual desire. Physical examination revealed wrinkles on the face, lack ofhair, short stature as well as small testicles. Hypergonadotropic hypogonadismand azoospermia were detected in the patient. The patient's karyotype analysiswas reported as 46 XX. SRY gene analysis was positive. These patients do nothave spermatogenesis, thus sperm can not be obtained with testis biopsy. Themain treatment of the disease is testosterone replacement therapy. Hastada hipergonadotropik hipogonadizm ve azospermisaptandı. Hastanın karyotip analizi 46 XX olarak rapor edildi. SRY gen analizipozitif bulundu. Bu hastalarda spermatogenez olmadığından testis biyopsisi ilesperm elde edilemez. Hastalığın başlıca tedavisi testosteron yerine koymatedavisidir. İnfertilite ile başvuran primer hipogonadizmli hastaların ayırıcıtanısında bu nadir hastalık dikkatealınmalıdır.

Thyrotoxicosis is a common condition associated with excess circulating thyroid hormones. Rarely, thyrotoxicosis can be developing due to functioning metastatic thyroid carcinoma. Most of patients with functional thyroid cancer metastases have had normal thyroxine (T4) concentrations but high serum triiodothyronine (T3-thyrotoxicosis). Inappropriately supressed TSH levels with normal T4 levels may be helpful to suspect T3 thyrotoxicosis. In this case report we present a case of thyrotoxicosis due to metastatic follicular thyroid cancer with a huge metastatic lesion.

Amaç: Adrenal insidentalomalar; adrenal bezle ilişkili herhangi bir hastalık kuşkusu yokken görüntüleme yöntemleri veya abdominal laparotomide tesadüfen saptanan kitlelerdir. Abdominal görüntüleme yöntemlerinin kullanımının artması ile adrenal insidentalomaların tespit edilme sıklığı da artmıştır. Bu çalışmada nonfonksiyonel adrenal insidentalomalı (NFAI) hastaların demografik özellikleri ve insülin direncini araştırmayı amaçladık.Yöntemler: Çalışmaya 2007-2013 yılları arasında Dicle Üniversitesi Tıp Fakültesi Endokrinoloji Anabilim Dalı'nda NFAI nedeniyle takip edilen 30 hasta (20 kadın, 10 erkek) ve 66 sağlıklı birey (44 kadın, 22 erkek) dahil edildi. Hasta dosyalarından hastalara ait fizik muayene, ultrason, bilgisayarlı tomografi sonuçları kaydedildi. Tüm hastalara 1 mg deksametazon testi, idrar metanefrin ve normetanefrin, serum aldosteron ve renin düzeyi bakılmıştı. Açlık kan şekeri (AKŞ) ve insülin değerleri mevcut olan hastaların homeostatic model assessment- insulin resistance (HOMA-IR) formülüne göre insülin direnci hesaplandı, yaş ve beden kitle indeksi (BKİ) farklı olmayan kontrol grubundaki sağlıklı bireylerle karşılaştırıldı.Bulgular: Hastaların yaş ortalaması 45,9±10,9, BKİ 28,5±5,8 kg/m2 idi. Gruplar arasında yaş, cinsiyet ve BKİ açısından fark yoktu. AKŞ ve HOMA IR, NFAI grubunda kontrol grubuna göre anlamlı olarak yüksekti (sırasıyla, p= 0,022 ve p=0,005). LDL, HDL kolesterol ve trigliserid düzeyleri bakımında gruplar arasında farklılık gözlenmedi (p>0,05). Sonuç: Nonfonksiyonel adrenal insidentalomalı hastalar insulin direnci ve metabolik parametreler açısından riskli olduklarından bu açıdan değerlendirilmelidir. Objective: Adrenal incidentaloma are bulks which are stated incidentally by imagining methods or in abdominal laparotomy when there is no suspicion of any disease in adrenal gland. With increase in using abdominal ultrasonography and BT, the frequency of determining adrenal incidentaloma also increases. In this study, we aimed to examine demographical features and insulin resistance in patients with non functioning adrenal incidentaloma (NFAI). Methods: 30 (20 female-10 male) patients, who followed due to NFAI at Dicle University Medical Faculty Department of Endocrinology between years of 2007-2013, and age, BMI matched 66 healthy subjects were included in the study. Results of physical examination, USG and BT were recorded from patients' files. All patients were underwent following analyses for excluding a functioning adrenal mass, overnight dexamethasone suppression test, 24 hour urinary metanephrine and normetanephrine, plasma aldosterone/ renin activity ratio. Insulin resistance was calculated in accordance with homeostatic model assessmentinsulin resistance formula. Results: The average of age was 45.9 ± 10.9 years and body mass ındex (BMI) 28.5 ± 5.8. There were no significantly difference in terms of age, gender and BMI between groups. Fasting blood glucose and insulin resistance were significantly higher in patients with non-functional adrenal incidentaloma than control group (p=0.022, p=0.005i respectively). No significant difference was found between groups, in terms of LDL, HDL and triglyceride levels. Conclusion: patients with NFAI are more prone to have insulin resistance and hyperglycemia. Thus, clinicians should evaluate those patients with NFAI, in terms of metabolic parameters.

Adrenal incidentalomas (AIs) are adrenal lesions detected on abdominal imaging procedures or abdominal laparotomy that had been performed for unrelated reasons. AIs have become a common clinical concern as a result of widespread use of abdominal ultrasound, CT and MR imaging in clinical practice. The aim of this study was to evaluate the demographical and clinical data our patients with AI. This study included data from 78 patients (53 female – 25 male) with AI those followed at Dicle University School of Medicine, Department of Endocrinology and Metabolism, between 2007 and 2013. Data collected from the records were physical and hormonal evaluation, diameter of the lesion, and the features of the lesion on radiological imaging. Also the results of serum cortisol, aldosterone, renin and 24 hour urinary metanephrine and normetanephrine were recorded. The average age of patients were 48.6 ±14.9 years. There was no significant difference in terms of localization of adenomas. Body mass ındex of patients were 28.54 ± 5.82. Of the patients, 45 (57.7%) were diagnosed as non-functional AI, 10 (12.8%) as subclinical Cushing’s syndrome, 11 (14,1%) as pheochromocytoma, 2(2.6%) as primary hyperaldosteronism, and 3(3.8%) as adrenal carcinoma. Ganglioneuroma was stated in 2 patients (2.6%), myelolipoma was stated in 1 patient (1.3%), adrenal adenoma was stated in 1 patient, adrenal cyst was stated in 1 patient, schwannoma was stated in 1 patient and neoplasia with small circular cells was stated in 1 patients. AIs are currently more frequently detected in clinical practice due to increased use of imaging techniques. Although the vast majority of AIs are benign and non-functioning, appropriate hormonal evaluation and imaging procedures must be performed. Radyolojik görüntüleme sıklığı arttıkça adrenal insidentaloma olgularının sayısı da artmaktadır. McCune-Albright syndrome is characterized by polyostatic fibrous dysplasia, brown spots on the skin (café au lait pigmentation) and autonomous endocrine hyperfunction. Early puberty and other endocrinological manifestations, such as acromegaly, gigantism and hypercortisolism are widely observed in the syndrome. Acromegaly is seen in 20% of patients. We report a case of acromegaly accompanied with this syndrome.Key words: McCune-Albright syndrome; acromegaly; fibrous dysplasia, McCune-Albright sendromu, poliostotik fibröz displazi, deride kahverenginde lekelenme (Cafe au lait lekeleri) ve otonomik endokrin hiperfonksiyon ile karakterize bir sendromdur. Allgrove syndrome is a very rare seen disorder with an autosomal recessive trait. The three characteristics of disease are alacrima, achalasia and adrenal failure. Alacrimia is the first manifestation in most cases. Achalasia and adrenal failure are seen later. Neurological involvement is rare and usually occurs in older age. A woman 22 years old admitted to our hospital with adrenal failure crisis. She had alacrimia and used teardrop for many years. Achalasia was diagnosed after evaluation of her dysphagia. Thus, diagnosis of Allgrove syndrome was done. Autonomic cardiac dysfunction and peripheral motor neuropathy were detected. In conclusion, Allgrove syndrome is rare and has life threatening potential due to adrenal failure. Early diagnosis and appropriate treatment may improve life quality and expectancy. Patients with relevant symptoms should be evaluated for autonomic neuropathy. Keywords: Allgrove; neuropathy; adrenal failure

Amaç: Bu çalışmada renal transplantasyon yapılmış, 18 yaşından büyük erişkinlerde böbrek nakli sonrası, hasta ve greft survilerinin değerlendirilmesi ve surviye etki eden faktörlerin ortaya çıkarılması amaçlanmıştır.Hastalar ve yöntem: İ.Ü. CTF Transplantasyon Ünitesi'nde Ocak 1990 ile Kasım 2010 tarihleri arasında böbrek nakli yapılan 494 hastanın arşiv kayıtları incelendi. 18 yaşından küçük veya dosyaları bulunamayan hastalar çalışmaya alınmadı. Hastaların nakil tarihi, doğum tarihi, cinsiyet, primer hastalığı, nakil öncesi yapılan RRT süresi ve türü, PRA düzeyi, HLA missmatch sayısı, verici türü, DM, HBV ve HCV durumu, alıcı boyu, kilosu ve VKİ, yaşı, verici cinsiyeti, verici kreatinin, kullandığı immunsupresif tedavisi, CMV proflaksisi, GGF, ilk 1 ay içinde gelişen AR, enfeksiyon, 1.ay, 3.ay, 1.yıl, 5.yıl ve son kontrolde kreatinin düzeyi, HT, DM, HPL durumu, proteinüri varlığı, greft kaybı varsa nedeni ve tarihi, hasta ölmüş ise nedeni ve tarihi dosyalarından elde edildi. Hasta ve greft sürvisi Kaplan-Meier yöntemiyle hesaplandı. Sürviye etkili olan faktörlerin çoklu değişkenli analizi cox regresyon analizi ile yapıldı.Bulgular: Hastalar genellikle orta yaşlı ve genç (ortalama yaş 32.3±10.1) erkeklerdi (%64). Nakiller yüksek (%82.6) oranda canlı vericilerden yapılmıştı. Kaplan-Meier yöntemine göre 1 ve 5 yıllık greft sürvisi; %94.6, %85, hasta sürvisi ise %98.5, %96.3'dü. Greft sürvisi üzerine 1.ay kreatinin, 1.yıl proteinüri, verici tipi, takrolimus kullanımı, mikofenolik asit türevi kullanımı ve nakil süreci anlamlı olarak etkili bulunmuştur. Bu etkili faktörler Cox regresyon analizinde değerlendirildiğinde takrolimus kullanımı, 1.ay kreatinin seviyesi ve nakil süreci anlamlılıklarını kaybetmiştir.Sonuç: Çalışmamızda CTF'deki renal transplantasyon sürvisini ortaya koyduk. Transplantasyon ile ilgili bu verilerin bilinmesi, özellikle canlı vericiden nakil yapılan hastalarda, hasta seçimi, canlı verici seçimi ve hasta bilgilendirilmesi için önemlidir. Objective: The purpose of this study is evaluation of patient and graft survival after renal transplantation in adult patients older than 18 years and determination of the possible factors which are affecting the survival.Patients and methods: Records of 494 transplant patients, grafted at Istanbul University Cerrahpasa Faculty of Medicine Renal Transplantation Unit between January 1990 and November 2010, were retrospectively analysed. Patients younger than 18 years and patients with missing data were excluded. Date of transplant, date of birth, gender, primary disease, duration and type of RRT applied pre-transplant period, panel reactive antibody (PRA) level, number of HLA missmatch, donor type, diabetes, HBV and HCV status, receiver height, weight and body mass index (BMI), age, donor gender, donor creatinine, used immunosuppressive therapy, CMV prophylaxis, GGF, AR occurring within the 1st month, infection, the level of creatinine at 1st and 3rd months, 1st year, 5th year period and the last visit, HT, DM, HPL status, presence of proteinuria, cause and date of graft loss, if the patient died, the cause and date was obtained from the patients' files. Kaplan-Meier method was utilized for the patient and geft survival analyses. Cox regression analysis were performed for multi-variable analysis of the factors which are affecting the survival.Results: The majority of patients were middle-aged and younger (mean age 32.3 ± 10.1) men (64%). About eighty two percent of the transplants were from living donors. One and five-year graft survival rates were 94.6%, 85%, respectively whereas for the same years overall survival rates were 98.5% and 96.3%, respectively. We demonstrated that 1st month creatinine level, 1st ar proteinüria, donor type, usage of tacrolimus, usage of micofenolic acid derivatives and date of transplant were associated with graft survival. We evaluated these factors by Cox regression analysis, Tacrolimus usage, 1st month creatinine level and date of transplantation lost their significance.Conclusion: In this study, we demonstrated the factors which are affecting kidney transplant survival. Knowledge of these data at pre and post transplantation period, especially for living donors is important for selection of patient, selection of living donor and ability to inform patient about transplantation. 47

Physical examination revealed wrinkles on the face, lack of hair, short stature as well as small testicles. Hypergonadotropic hypogonadism and azoospermia were detected in the patient. The patient's karyotype analysis was reported as 46 XX. SRY gene analysis was positive. These patients do not have spermatogenesis, thus sperm can not be obtained with testis biopsy. The main treatment of the disease is testosterone replacement therapy. In the differential diagnosis of patients with primary hypogonadism presenting with infertility, this rare disease should be considered.
