The brachial plexus consists of the ventral twigs of the last four cervical nerves and the first thoracic nerve. It ensures the motor and sensitive innervations of the thoracic limb.Our goal was to describe the brachial plexus of the cervical region to the middle third of the arm.We conducted a prospective study at the anatomy Laboratory of the Faculty of Medicine and Dentistry in Bamako from September 2016 to October 2017. We dissected the brachial plexus (PB) of 13 fresh adult corpses on both sides. The inclusion criteria were: Fresh adult corpses with cervical regions and brachial without scarring. The injected or scar-carrying corpses were not included in the cervical and brachial regions.Twenty-six brachial plexus of which 18 bp in men and 8 bp in women were dissected. The average age of the subjects was 42 years (extreme: 18 and 70 years). We noted nerve block variations in 3.8%, fascicular in 3.8% and late terminal in 73.1%. The involvement of the anterior branch of the fourth spinal nerve (C4) was found in 46.2%.The brachial plexus is the seat of many anatomical variations whose knowledge is indispensable to treat its lesions.Le plexus brachial est constitué des rameaux ventraux des quatre derniers nerfs cervicaux et du premier nerf thoracique. Il assure l'innervation motrice et sensitive du membre thoracique.Notre but était de décrire le plexus brachial de la région cervicale au tiers moyen du bras.nous avons réalisé une étude prospective au laboratoire d'anatomie de la Faculté de Médecine et d'Odontostomatologie de Bamako de septembre 2016 à octobre 2017. Nous avons disséqué des deux côtés les plexus brachiaux (PB)de 13 cadavres frais d'adultes. Les critères d'inclusion étaient : cadavres frais d'adulte avec les régions cervicale et brachiale sans cicatrice. N'ont pas été inclus les cadavres injectés ou porteurs de cicatrice au niveau des régions cervicale et brachiale.Vingt-six plexus brachiaux dont 18 PB chez les hommes et 8 PB chez les femmes ont été disséqués. L'âge moyen des sujets était de 42 ans (extrême : 18 et 70 ans). Nous avons noté des variations tronculaires dans 3,8%, fasciculaires dans 3,8% et en fin terminales dans 73,1%. La participation du rameau antérieur du quatrième nerf spinal cervical (C4) a été trouvée dans 46,2%.Le plexus brachial est le siège de nombreuses variations anatomiques dont la connaissance est indispensable pour traiter ses lésions.

To describe the epidemiological, clinical and therapeutic aspects of acute surgical abdomens at the Koutiala Reference Health Center.This was a prospective and descriptive study from August 1, 2017 to May 31, 2018. It included all patients operated on for acute abdomen in the General Surgery Department of the Koutiala Reference Health Center. The study did not include the nonoperated surgical acute abdomens, the non-surgical acute abdomens. Clavien-Dindo classification was used to evaluate complications.One hundred patients were registered. Acute surgical abdomens accounted for 8.4% of consultations (n = 1190), 27.7% of surgical procedures (n = 361) and 70% of surgical emergencies (n = 142). The average age was 34.4 years ± 18.5. Men were in the majority with 70%. The mean duration of change in symptomatology was 4.8 days ± 5.4. Patients consulted for abdominal pain (100%), fever (60%), vomiting (88%) and discontinuation of materials and gases (32%). The physical examination noted abdominal distension (53%), abdominal contracture (36%), abdominal defense (56%) and pain in the cul de sac of Douglas (95%). X-rays of the abdomen without preparation and abdominopelvic ultrasonography contributed to the diagnosis in 46% of cases and 18% of cases, respectively. Acute appendicitis (35%) was the most common etiology followed by acute peritonitis (31%) and acute intestinal obstruction (15%). We performed an appendectomy in 45 patients (45%), resection anastomosis (15%) and excision-suture (13%). The average duration of hospitalization was 4.7 days. The morbidity was 12%. According to the Clavien-Dindo classification, 9 patients were grade III and 3 grade V. The mortality was 3%.Acute surgical abdomens are the most common emergencies in our practice. The diagnosis is clinical and para-clinical most often. Morbidity and mortality remain elevated. The outcome of treatment depends on early management and mastery of abdominal surgery techniques.Décrire les aspects épidémiologiques, cliniques et thérapeutiques des abdomens aigus chirurgicaux au Centre de Santé de Référence de Koutiala.Il s'agissait d'une étude prospective et descriptive allant du 1er Aout 2017 au 31 Mai 2018. Elle a porté sur tous les patients opérés pour abdomen aigu dans le service de chirurgie générale du centre de santé de référence de Koutiala. N'ont pas été retenus dans notre étude les abdomens aigus chirurgicaux non opérés, les abdomens aigus non chirurgicaux. La classification de Clavien-Dindo a été utilisée pour évaluer les complications.Cent patients ont été enregistrés. Les abdomens aigus chirurgicaux ont représenté 8,4% des consultations (n=1190), 27,7% des interventions chirurgicales (n=361) et 70% des urgences chirurgicales (n=142). L'âge moyen était de 34,4 ans±18,5. Les hommes étaient majoritaires avec 70%. La durée moyenne d'évolution de la symptomatologie était de 4,8 jours±5,4. Les patients ont consulté pour douleur abdominale (100%), la fièvre (60%), les vomissements (88%) et l'arrêt des matières et des gaz (32%). L'examen physique a noté la distension abdominale (53%), la contracture abdominale(36%), la défense abdominale (56%) et la douleur au cul de sac de Douglas (95%). La radiographie de l'abdomen sans préparation et l'échographie abdominopelvienne ont contribué respectivement au diagnostic dans 46% des cas et 18% des cas. Les appendicites aigues(35%) étaient les étiologies les plus fréquentes suivies des péritonites aigues (31%) et les occlusions intestinales aigues (15%). Nous avons réalisé une appendicectomie chez 45 patients (45%), la résection anastomose (15%) et l'excision-suture (13%). La durée moyenne d'hospitalisation était de 4,7 jours. La morbidité était de 12%. Selon la classification de Clavien-Dindo, 9 patients étaient de grade III et 3 de grade V. La mortalité était de 3%.Les abdomens aigus chirurgicaux sont des urgences les plus fréquentes dans notre pratique. Le diagnostic est clinique et para clinique le plus souvent. La morbi-mortalité reste élever. Le résultat du traitement dépend de la prise en charge précoce et la maitrise des techniques de chirurgie abdominale.

the aim of this study was to evaluate the clinical and therapeutic aspects of non-traumatic digestive perforations at the Koutiala Reference Health Center.This was a prospective and descriptive study from August 1, 2017 to December 31, 2018. Patients admitted and operated on for non-traumatic digestive perforation were included. The parameters studied were age, sex, frequency, clinical aspects, etiologies, treatment and operative follow-up.Sixty-one patients were registered. Non-traumatic digestive perforations accounted for 78.2% of cases of acute peritonitis (n = 78). Men were in the majority with 73.8%. The average age was 34.5 years old. The symptomatology was represented by abdominal pain in all patients, vomiting in 56 patients and fever in 42 patients. The mean duration of evolution of the symptomatology was 5.5 days. On physical examination, the most common signs were abdominal contracture with 81.9%, disappearance of prehepatic maturation (52.4%) and pain in rectal examination (95.1%). Radiological pneumoperitoneum was found in 39 patients. The serodiagnosis of Widal was positive in 15 cases. The etiologies were dominated by perforation of infectious origin with 77.0% (46). The perforation was ileal in 28 patients, appendicular in 18 patients and gastroduodenal in 11 patients. Excision-suturing of the perforation was done in 30 patients, resection anastomosis in 8 patients and appendectomy in 18 patients. Morbidity and mortality were respectively 14.7% and 6.5%.Non-traumatic digestive perforations are the first cause of peritonitis in our department. The ileal seat is the most common and is most often secondary to typhoid fever. Morbidity and mortality remain high and this result is a reflection of late diagnosis which is the main factor of severity of digestive perforations.Evaluer les aspects cliniques et thérapeutiques des perforations digestives non traumatiques au Centre de Santé de Référence de Koutiala.Il s'agissait d'une étude prospective et descriptive allant du 1erAoût 2017 au 31 décembre 2018. Les patients admis et opérés pour perforation digestive non traumatique étaient inclus. Les paramètres étudiés étaient l'âge, le sexe, la fréquence, les aspects cliniques, les étiologies, le traitement et les suites opératoires.Soixante-un patients ont été enregistrés. Les perforations digestives non traumatiques ont représenté78,2% de causes de péritonite aigue (n=78). Les hommes étaient majoritaires avec 73,8%.L'âge moyen était de 34,5 ans. La symptomatologie était représentée par la douleur abdominale chez tous les patients, les vomissements chez 56 patients etla fièvre chez 42 patients. La durée moyenne d'évolution de la symptomatologie était de 5,5 jours. A l'examen physique, les signes les plus fréquents étaient la contracture abdominale avec 81,9%, la disparition de la matité pré hépatique (52,4%) et la douleur au toucher rectal (95,1%). Un pneumopéritoine radiologique a été trouvé chez 39 patients. Le sérodiagnostic de Widal était positif dans 15 cas. Les étiologies étaient dominées par la perforation d'origine infectieuseavec77,0% (46). La perforation était iléal chez 28 patients, appendiculaire chez 18 patients et gastroduodénale chez 11 patients. L'excision-suture de la perforation a été faite chez 30 patients, la résection anastomose chez 8 patients et l'appendicectomiechez18 patients. La morbidité et la mortalité ont été respectivement de 14,7%de 6,5%.Les perforations digestives non traumatiques sont la première cause de péritonite dans notre service. Le siège iléal est le plus fréquent et qui est secondaire le plus souvent à la fièvre typhoïde. La morbi-mortalité reste élever et ce résultat est le reflet du diagnostic tardif qui constitue le principal facteur de gravité des perforations digestives.

Nephroblastoma or Wilms tumor is a malignant tumor that develops at the expense of embryonic kidney tissue. It is the most common kidney tumor in children (> 90%); it represents around 5% to 14% of all childhood cancers. The aim of our work is to recall the contribution of imaging, in particular CT, in the management of this renal tumor. This is a retrospective, single-center study of 136 cases of Wilms tumors diagnosed in our clinic over a 2-year period. The mean age of the patients was 04 years with an extreme of (3 months -12 years) with a female prevalence (8 girls for 5 boys). Abdominal mass dominated in our study in clinical information. Histological confirmation was found in 12 cases, the only non-conforming case was lost to follow-up.

9 pages; International audience; Particularly dry or wet boreal summer monsoon seasons are major hazards affecting societal vulnerability in India and Africa. Several factors affect monsoon rainfall amount and limit the understanding of possible linkages between monsoon variability and mean climate changes. Here we characterize the multiscale variability of Indian and West African monsoon rain from two simulations of the last 6,000 years. Changes in Earth's orbit cause long‐term monsoon drying trend in India and Africa, but the Indian monsoon is more sensitive to anthropogenic CO2. Variability is characterized by two major ranges of chaotic variability, each related to specific ocean‐atmosphere modes present throughout the period. Combination of random 50‐ to 500‐ and 2‐ to 20‐year variability leads to large events occurring at millennium scale. However, the two regions exhibit opposite trends in rainfall variability due to changes in teleconnection with Pacific sea surface temperature for India and Atlantic sea surface temperature for West Africa at interannual to decadal timescales.

Field experiments were conducted to determine the relationship of tomato anthracnose to weather variables. Sixteen potted tomato plants were exposed to field conditions within rows of tomato plants for 4 consecutive days at various time periods during the 1993 and 1994 summer growing seasons. Incidence of fruit infection by Colletotrichum coccodes was correlated with rain variables (amount and duration of rain) alone and in combination with other meteorological factors. The best fitting regression equation, accounting for 72% of the variation in anthracnose incidence (arcsine-square root transformed), was Y = 111.77 - 1.16 HNRo, in which HNRo is the numbers of hours during which no rainfall occurs within 4-day intervals that tomato fruit were exposed to field conditions in central Pennsylvania.

Tomato fruit grown for commercial processing are harvested when the majority of the fruit are at the full, red-ripe stage of development. At this physiological stage, marketable yields often are reduced significantly by Colletotrichum coccodes. Appressorium formation and the infection of tomato fruit by C. coccodes were investigated in controlled-environment experiments. Conidia of C. coccodes were subjected to five temperature treatments (10 to 34°C with 6°C increments), and eight incubation periods (3 to 24 h with 3-h increments). The highest proportion of conidia that formed appressoria occurred at 16 and 22°C. Appressoria were formed within as few as 6 h of incubation at 16, 22, and 28°C. In contrast, incubation periods of at least 15 and 18 h were required for appressoria to form at 34 and 10°C, respectively. Appressorium formation was significantly reduced by 0.1 to 0.2 ppm of the fungicide chlorothalonil, and no appressoria formed at concentrations >0.4 ppm. When tomato fruit were inoculated with C. coccodes at three inoculum densities (2 × 105, 6 × 105, and 10 × 105 conidia/ml) and incubated in dew chambers for 8, 16, and 24 h at 5°C increments from 15 to 35°C, there was no significant interaction among inoculum density, dew period, and temperature. In general, across all inoculum densities and dew periods, anthracnose severity levels were greater for each 5°C increase in temperature from 15°C until its maximum level was observed at 30°C. However, when the fruit were exposed to 35°C, disease development was minimal. At temperatures from 15 to 30°C anthracnose severity increased proportionally as dew-period duration and inoculum density increased.

Cette étude consiste à la modélisation et à l’application de méthodes numériques afin d’optimiser les performances d’un capteur solaire plan à eau sous les conditions météorologiques du Mali, particulièrement au mois de janvier ou l’ensoleillement est faible et la consommation en eau chaude importante. Les résultats obtenus montrent que lorsque la vitesse du vent passe de 1 m/s à 4 m/s le rendement décroit de 69 % à 65 % pour une intensité du rayonnement solaire de 460 W/m2. L’augmentation de l’épaisseur de l’isolant, des diamètres intérieur et extérieur des tubes donne un rendement pouvant atteindre respectivement 67 % et 70 %. Lorsque le matériau constituant l’absorbeur, la vitre et l’isolant sont respectivement en acier chrome, en poly-méthacrylate, en mousse de polyuréthane, on obtient des rendements de 71 %, 61,5 % et 52,5 % respectivement pour une intensité du rayonnement solaire de 460 W/m2. Aussi, cette étude a montré que les paramètres externes tels que le rayonnement solaire, la vitesse du vent et internes (propriétés optiques et thermo-physiques des différents composants du capteur) influent très peu sur la température du fluide caloporteur.

The paper aimed at assessing the capabilities and limitations of five different precipitation products to describe rainfall over West Africa. Five gridded precipitation datasets of the Tropical Rainfall Measurement Mission (TRMM) Multi-Platform Analysis (TMPA 3B43v7); University of Delaware (UDEL version 3.01); Climatic Research Unit (CRU version 3.1); Global Precipitation Climatology Centre (GPCC version 7) and African Rainfall Climatology (ARC version 2) were compared and validated with reference ground observation data from 81 stations spanning a 19-year period, from January 1990 to December 2008. Spatial investigation of the precipitation datasets was performed, and their capability to replicate the inter-annual and intra-seasonal variability was also assessed. The ability of the products to capture the El Nino and La Nina events were also assessed. Results show that all the five datasets depicted similar spatial distribution of mean rainfall climatology, although differences exist in the total rainfall amount for each precipitation dataset. Further analysis shows that the three distinct phases of the mean annual cycle of the West Africa Monsoon precipitation were well captured by the datasets. However, CRU, GPCC and UDEL failed to capture the little dry season in the month of August while UDEL and GPCC underestimated rainfall amount in the Sahel region. Results of the inter-annual precipitation anomalies shows that ARC2 fail to capture about 46% of the observed variability while the other four datasets exhibits a greater performance (r > 0.9). All the precipitation dataset except ARC2 were consistent with the ground observation in capturing the dry and wet conditions associated with El Nino and La Nina events, respectively. ARC2 tends to overestimate the El Nino event and failed to capture the La Nina event in all the years considered. In general GPCC, CRU and TRMM were found to be the most outstanding datasets and can, therefore, be used for precipitation assessment over West Africa.

Introduction L’exophtalmie est souvent l’expression de la maladie de Basedow, secondaire a une infiltration inflammatoire des muscles et du tissu conjonctif orbitaire. L’augmentation de la graisse orbitaire peut etre associee au syndrome de Cushing et a l’insulinoresistance [1] , [2] . Observations Patiente avec exophtalmie bilaterale (protrusion 21,2 mm a droite et 28 mm a gauche), presente une obesite morbide avec un index de masse corporelle a 57,82 kg/m2. L’examen ophtalmologique objective un score EUGOGO a 3-4. Le scanner orbitaire, les dosages hormonaux et immunologiques ont exclu : une thyropathie auto-immune, le syndrome de Cushing, ainsi que d’autres pathologies orbitaires ou cerebrales. La seule anomalie en imagerie etait l’expansion du tissu adipeux de l’orbite avec conservation de l’aspect souple des muscles. Un traitement par cure de solumedrol a ete debute, associe avec regime hypocalorique et exercice physique, le traitement par metformine a ete discute. Discussion Parmi les causes d’exophtalmie, il faut inclure l’insulinoresistance, qui devient de plus en plus frequente avec la croissance de l’incidence de l’obesite.

Introduction L’acromegalie est une maladie rare de diagnostic tardif [1] . Nous rapportons un cas de decouverte precoce. Observation Patient de 55 ans, travailleur manuel, diabetique de type 1 depuis 14 ans, sous pompe a insuline, admis pour diabete desequilibre avec hyperglycemies nocturnes malgre une bonne dietetique. Pas de plaintes fonctionnelles. Pas de syndrome dysmorphique facial franc comparativement aux photographies anterieures. IGF1 320 ng/mL (limite 210), GH 13,3 ng/mL (borne IRM cerebrale : adenome latero-sellaire droit centimetrique, a extension inferieure sans signe d’envahissement. Le patient est adresse pour chirurgie hypophysaire. Discussion L’acromegalie entraine une augmentation de la neoglucogenese et une resistance a l’insuline [2] , [3] . En l’absence de syndrome dysmorphique classique, l’analyse minutieuse des courbes glycemiques pourrait rendre le diagnostic de l’acromegalie plus precoce et ameliorer ainsi sa prise en charge.

Introduction Les prolactinomes sont les adenomes hypophysaires les plus frequents (60 %) ; le sex-ratio est de 10 femmes pour un homme avant 50 ans. Les macroprolactinomes sont plus frequents chez l’homme. La transformation d’un prolactinome en carcinome hypophysaire est exceptionnelle, une mutation du gene RAS a ete incriminee dans certaines etudes. Observation Nous rapportons un cas d’adenome a prolactine kystique, apparemment isole et benin dont l’anatomopathologie revele la presence d’une lesion malpighienne de haute malignite. Monsieur M.L., 28 ans nous a ete adresse par le service d’imagerie, pour une masse intrasellaire de 13 mm de diametre et 18 mm de hauteur avec expansion suprasellaire, d’allure kystique responsable d’une latero-deviation de la tige pituitaire a l’IRM hypophysaire qui avait ete demandee par son medecin traitant pour des douleurs retro-orbitaires evoluant depuis un an. A l’examen clinique : TA : 11/8 cmHg, poids : 73 kg, IMC : 25,63 kg/m2, dysfonction erectile. L’hypophysogramme montre une hyperprolactinemie isolee (prolactine : 789 mUI/L). Le champ visuel etait normal. Le patient a ete opere par voie trans-sphenoidale et a presente en postoperatoire un diabete insipide et une hypothyroidie centrale qui ont ete supplementes. L’anatomopathologie de la piece operatoire montre : un adenome hypophysaire a prolactine avec une surexpression de la proteine P53 et une seconde lesion epitheliale malpigienne qui exprime la CK 5/6, avec un index de proliferation estime par l’anticorps Ki67 superieur a 80 %. Discussion Le carcinome hypophysaire est rare, sa prise en charge est mal codifiee et l’evolution est le plus souvent defavorable.

Determine prevalence of the depression in a population of patients chronic hemodialysis. Look for the associated sociodemographic, clinical and biological factors.It was a transverse study. She concerned the at least 18-years-old patients, followed in the center of Hemodialysis of the CHU at the rate of 2 - 3 sessions a week. The patients having agreed, were in dialysis at least for 3 months and benefited from an individual interview through an index card of investigation based of Hamilton's scale French version in 17 items.On a sample of 107 patients, the average age was 43, 24 ± 14 years old with extremes between 20 and 75 years. The average duration in dialysis was of ±40, 37 36, 5 months with extremes between 4 and 144 months. Prevalence of the depression was 88 %. The depression was light in 78, 5 %, moderated in 6,5% and severe in 3 %. The anemia was present at 94 patient's among whom 76 had a light depression.We observed that prevalence of the depression remains high to the chronic hemodialysis.Déterminer la prévalence de la dépression dans une population de patients hémodialysés chroniques. Rechercher les facteurs sociodémographiques, cliniques et biologiques.Il s'est agi d'une étude transversale. Elle a concerné les patients âgés d'au moins 18 ans, suivis au Centre d'Hémodialyse à raison de 2 – 3 séances par semaine. Les patients ayant acceptés, étaient en dialyse au moins depuis 3 mois et ont bénéficiés un entretien individuel à travers une fiche d'enquête basé sur l'échelle d'Hamilton version française à 17 items.Sur un échantillon de 107 patients, l'âge moyen était de 43,24 ± 14 ans avec les extrêmes entre 20 et 75 ans. La durée moyenne en dialyse était de 40,37 ±36,5 mois avec les extrêmes entre 4 et 144 mois. La prévalence de la dépression était de 88 %. La dépression était légère dans 78,5 %, modéré dans 6,5 % et sévère dans 3 %. L'anémie était présente chez 94 patients dont 76 avaient une dépression légère.Nous avons observé que la prévalence de la dépression reste élevée chez les hémodialysés chroniques.

To study the therapeutic aspects and prognosis of volvulus of the sigmoid colon in surgical departments.Our study was retrospective from January 2000 to December 2009 (10 years). All patients who underwent sigmoid volvulus in the Point G teaching hospital surgical departments were taken into account.We recorded 96 patients operated for volvulus of sigmoid over 882 cases of bowel obstruction, a rate of 10.9%. The average age was 47.2 years ± 18.9. In per-operative, there was a necrosis of the colon rate of 16.7% (14 cases). Performed surgical treatment modalities were: sigmoidectomy with primary anastomosis in 29.2% of cases (28), sigmoidectomy followed by colostomy and secondary anastomosis in 60.4% of patients (58) and 10.4% rare of simple devolvulation. The immediate postoperative period were marked by a morbidity rate of 7.3% (7 cases) and a mortality rate of 7.3% (7 cases).In spite of various therapeutic modalities of volvulus of the sigmoid colon, the rates of postoperative morbidity and mortality are rising in the general surgery of the Point G teaching Hospital.Le but était de décrire les aspects thérapeutiques et de déterminer le pronostic du volvulus du côlon sigmoïde dans les services de chirurgie générale.Notre étude a été rétrospective allant de janvier 2000 à décembre 2009 (soit 10 ans). Tous les patients opérés dans les services de chirurgie générale du CHU du Point G pour volvulus du sigmoïde ont été pris en compte.Nous avons enregistré 96 patients opérés pour volvulus du sigmoïde sur 882 cas d'occlusion intestinale soit une fréquence de 10,9%. La moyenne d'âge était de 47,2 ans ±18,9 ans. En per-opératoire, il y avait une nécrose de l'anse volvulée chez 16,7% (14 cas). Les modalités thérapeutiques chirurgicales effectuées ont été: la sigmoïdectomie avec anastomose immédiate dans 29,2% (28 cas), la sigmoïdectomie suivie de colostomie puis anastomose secondaire dans 60,4% (58 cas) et la dévolvulation simple dans 10,4% (10). Les suites opératoires immédiates ont été marquées par un taux de morbidité de 7,3% (7) et un taux de mortalité de 7,3% (7).En dépit des modalités thérapeutiques variées du volvulus du côlon sigmoïde, les taux de morbidité et de mortalité postopératoires restent élevés dans les services de chirurgie générale du CHU du Point G.

To determine the incidence of postoperative peritonitis, and describe their clinical, paraclinical and therapeutic aspects.This was a retrospective study conducted between 1980 and 2010 in the Department of Surgery B of the Point G University Hospital. It focused on all patients who underwent surgical intra-abdominal in the Surgery B department.We collected 25 cases of postoperative peritonitis, 84% were male subjects. The average age of patients was 37.2 years ± 17.0. The clinical picture was dominated by abdominal pain (92%), abdominal defense (92%), and fever (84%). Abdominal ultrasound played an important role in diagnosing 72% of cases. Etiologies were dominated by anastomotic leak (32%), infection of the collection phrenic, iatrogenic perforation (28%). Interventions made at surgery were: suture of the perforation or anastomotic recovery plus peritoneal cleansing and drainage in 36% of cases, peritoneal cleansing and drainage in 36%. There were postoperative complications in five patients. The mortality rate was 4%.Postoperative peritonitis are rare in our service. When they occur their rate of morbidity and mortality are high.Les objectifs étaient de déterminer la fréquence des péritonites postopératoires, de décrire leurs aspects cliniques, paracliniques et thérapeutiques.Il s'agissait d'une étude rétrospective réalisée entre 1980 et 2010 dans le service de de chirurgie B du CHU du Point G.Elle a porté sur tous les malades hospitalisés ayant subi une intervention chirurgicale intra abdominale en chirurgie B.Nous avons colligés 25 cas de péritonites postopératoires dont 84% de sujet masculin. L'âge moyen des patients a été de 37,2 ans ±17,0. Le tableau clinique était dominé par la douleur abdominale (92%), la défense abdominale (92%), la fièvre (84. L'échographie abdominale a contribué au diagnostic dans 72% des cas. Les étiologies étaient dominées par la désunion anastomotique (32%), la collection infectée sous phrénique, la perforation iatrogène (28%). Les gestes réalisés à l'intervention chirurgicale étaient la suture de la perforation ou la reprise de l'anastomose plus une toilette péritonéale et un drainage dans 36%, la toilette péritonéale et drainage dans 36%. Les suites opératoires ont été compliquées chez cinq malades. La mortalité a été de 4%.Les péritonites postopératoires sont rares dans notre service. Quand elles surviennent leurs taux de morbidité et de mortalité sont élevés.

Objective : A monthly sampling of benthic macroinvertebrates was done at the hydro-agricultural of dam Boura in the basin of the Volta (Burkina) during the period of February to July 2012 in order to describe the structuring of insect succession along with changes occurring in habitats of this irrigation system. Methodology and results : The samples of insects were collected from 6 stations placed on the shoreline (Station I), the adjoining zone to the coastline (Station II), the sewage channal (stations III, IV that is located at midsection of the irrigated area and the Station V far away from this area) and the Mouhoun stream (Station VI); the sampling method used conventional method the European Union named " Directive Cadre sur l’Eau (DCE) " advised for the sampling of the benthics macroinvertebrates. The survey revealed a community composed in majority of insects (more of 75%) and sparsely distributed. On one hand at the shoreline and the coastal- adjoining zone in the lake of dam, 23 families of macroinvertebrates were sampled; they mostly belong to the shoreline except for 10 families identified in the adjoining zone to the coastline. These two zones of the dam shelter the same malacological fauna consisting of the families of Unionidae, Lymneidas, Ampullariidae, Planorbidae, Valvatidae and Bulinidae. The survey revealed otherwise that the difference between the biological diversity indices of Shannon of these two zones was more pronounced during the month of July, period of rain in the basin., concerning the dam-sewage channal-river continuum, a total of 35 families were sampled: 27 from dam water and stations near the irrigated zones; and 32 been collected from the station of the sewage channel far from the irrigated perimeter and from the stream. The identification of individuals belonging to the family of the Baetidae and the Ephemerellidae (order of the Ephemeroptera) in the river water highlights a possible reconstruction of biodiversity in the river because both two families are indicator fertile water. Conclusion and application of results : it is concluded of this survey that the agricultural hydro amenities have an influence on the spatio-temporal structuring of the macroinvertebrates. The results of this survey can be used otherwise to keep biodiversity and the restoration of these ecosystems continually impacted. Two types of germination were exhibited by Colletotrichum coccodes when sclerotia, placed on moist filter papers in petri plates that were then either not sealed or sealed with Parafilm, were incubated for 5 days at temperatures of 10 to 34°C under a 14-h photoperiod. In sealed plates, germination was primarily myceliogenic; in nonsealed plates, sclerotial germination was only sporogenic. Setose, slightly orange-colored conidial masses were profusely produced on sclerotia at all temperatures greater than 10°C. Hyaline masses of conidia with sparse setae were produced on sclerotia maintained in complete darkness, and no mycelia formed in the sealed plates. In all temperature treatments, the numbers of colony-forming units per 100 sclerotia were greater in nonsealed than in sealed plates and greater under light than in complete darkness.

La crise mondiale du riz survenue en 2008 a montre l’imperative necessite pour les pays tributaires des importations comme la Cote d’Ivoire, d’assurer leur autosuffisance. La fertilisation minerale figure parmi les cinq axes strategiques definis pour l’accroissement de la production rizicole en Cote d’Ivoire. Sachant l’effet stimulateur de l’engrais azote sur le developpement de certaines maladies telle que la Pyriculariose du riz, un essai a ete conduit dans le but de savoir l’influence d’une application optimale de l’azote sur l’epidemie de la Panachure jaune du riz ou Rice Yellow Mottle Virus (RYMV). L’etude a ete realisee selon un dispositif experimental en SPLIT-PLOT comportant quatre niveaux d’azote (0 kg N Ha-1, 30 kg N Ha-1, 60 kg N Ha-1, 120 kg N Ha-1), trois niveaux de phosphore (0 kg P Ha-1, 13 kg P Ha-1, 26 kg N Ha-1) et 10 genotypes de riz irrigue. Les resultats ont montre que le phosphore et l’interaction NxP n’ont aucun effet significatif sur l’epidemie de RYMV. A l’oppose, les analyses ont mis en evidence un effet significatif de l’azote sur l’incidence de la Panachure jaune a des doses comprises entre 0 et 30 Kg N Ha-1. Cet intervalle de doses etant en deca de celui des doses recommandees en Cote d’Ivoire, l’intensification de la riziculture aquatique par une fertilisation azotee optimale, n’aura pas d’incidence significative sur le developpement de la Panachure jaune. Il faudra cependant sensibiliser les riziculteurs sur l’interet d’appliquer des doses d’engrais recommandees dans la lutte contre la virose. The chile pepper (Capsicum annuum) crop is affected by several pests, pathogens, and weeds including Verticillium dahliae, Meloidogyne incognita, spurred anoda (Anoda cristata), Wright groundcherry (Physalis acutifolia), and tall morningglory (Ipomoea purpurea). These weed species are unimpaired hosts to V. dahliae and M. incognita. Chile plants have been found co-infected with V. dahliae and M. incognita in commercial fields. Greenhouse studies were conducted to determine the relationships among V. dahliae, M. incognita, and each of the four aforementioned plant species. Plants were either non-inoculated or inoculated with V. dahliae, M. incognita, or V. dahliae plus M. incognita. Six weeks after inoculation, plant infection by V. dahliae, M. incognita reproduction, plant height and biomass were measured. Three relationships were identified: V. dahliae was recovered from 100% of all four inoculated plant species, irrespective of M. incognita treatment; V. dahliae and M. incognita enhanced or had no effect on weed biomass but were pathogenic to chile; and co-infection by V. dahliae had no effect on nematode reproduction in the first M. incognita generation on the crop or weeds. These biological relationships suggest that the competitive impact of the weeds may increase and pathogen diversity may be affected in infested fields, ultimately impacting the efficacy of our common IPM tools. Sanogo, S., and Yang, X. B. 2004. Overview of selected multivariate statistical methods and their use in phytopathological research. Phytopathology 94:1004-1006. To disentangle the nature of a pathosystem or a component of the system such as disease epidemics for descriptive or predictive purposes, mensuration is conducted on several variables of the physical and chemical environment, pathogenic populations, and host plants. For instance, it may be desired to (i) distinguish pathogenic variation among several isolates of a pathogen based on disease severity; (ii) identify the most important variables that characterize the structure of an epidemic; and (iii) assess the potential of developing regional scale versus site-specific postmanagement schemes using weather and site variation. In all these cases, a simultaneous handling of several variables is required, and entails the use of multivariate statistics such as discriminant analysis, multivariate analysis of variance, correspondence analysis, and canonical correlation analysis. These tools have been used to varying degree in the phytopathological literature. A succinct overview of these tools is presented with cited examples.

La riziculture pluviale, en zone forestiere de la Cote d'Ivoire, est confrontee a plusieurs contraintes, dont, notamment la baisse de fertilite du sol et l'enherbement excessif. L'effet du paillage de Cajanus cajan comme engrais organique pour le riz pluvial, NERICA 1(Bonfani), a ete evalue, en termes de rendement du riz. Cette etude a ete conduite de 2003 a 2005 a la station de recherche du Centre National de Recherche Agronomique (CNRA) de Gagnoa. Deux rotations, a base de riz, avec les jacheres a C. cajan de 6 mois et de 12 mois ont ete testees et comparees, a celle avec la jachere naturelle. La biomasse des precedentes jacheres est restee sur le sol comme mulch. Les resultats montrent que C. cajan âge de 12 mois a produit une biomasse seche de 17 t ha-1, en moyenne sur deux ans contre 1,7 t.ha-1 en 6 mois. La rotation riz avec C. cajan de 12 mois a ameliore significativement le rendement du riz, avec un gain de paddy qui a varie entre 700 et 900 kg ha-1, par rapport a la jachere naturelle. En plus, cette jachere a cajanus de 12 mois a permis de maintenir un minimum de rendement 1t ha-1. Cette technique offre une alternative de gestion durable des systemes traditionnels de riziculture pluviale.

Production and germination of conidia of Trichoderma stromaticum, a mycoparasite of Crinipellis perniciosa on cacao. Phytopathology 92:1032-1037. Growth characteristics of the fungus Trichoderma stromaticum, a mycoparasite on the mycelium and fruiting bodies of Crinipellis perniciosa, the causal agent of witches’-broom disease of cacao, were evaluated under controlled environmental conditions. The ability of T. stromaticum to produce conidia and germinate on dry brooms was evaluated at three constant temperatures (20, 25, and 30°C) and two constant relative humidities (75 and 100%). T. stromaticum produced abundant conidia on brooms at 100% relative humidity and incubation temperatures of 20 and 25°C, but none at 30°C. Sporulation of T. stromaticum was not observed at 75% relative humidity at any temperature. At 100% relative humidity and either at 20 or 25°C, treatment of brooms with T. stromaticum suppressed C. perniciosa within 7 days. In contrast, at 30°C, treatment with T. stromaticum had no effect on the pathogen in brooms maintained at either 75 or 100% relative humidity. Mycelium of C. perniciosa grew from brooms at all temperatures at 100% relative humidity. Conidial germination on broom tissue approximated 80% at temperatures from 20 to 30°C. Results suggest that applying T. stromaticum under high-moisture conditions when the air temperature is below 30°C may enhance the establishment of this mycoparasite in cacao plantations. The average infection efficiency of ascospores of Venturia inaequalis deposited on cluster leaves of apple flower buds was 6 to 16%, 3 to 9%, and 0.4 to 0.6% at tight cluster, first pink, and full pink-to-bloom, respectively. No lesions were observed on flower bud cluster leaves at petal fall. However, the leaves on the vegetative shoot emerging from the flower bud were highly susceptible; the average infection efficiency of ascospores on these leaves was 6 to 21%. The infection efficiency was more variable on young cluster and vegetative shoot leaves than on developing and mature cluster leaves. Results from this study indicate that differences in infection efficiency of V. inaequalis ascospores could be identified by apple bud growth stages.

Une etude des macroinvertebres dans deux lacs de barrages hydro- agricoles (Bama et de Boura) et dans le lac naturel de Bala a permis de mettre en evidence la distribution spatio-temporelle des macroinvertebres bioindicateurs de la qualite des eaux. La collecte des echantillons a ete inspiree de la methode conventionnelle de la Directive Cadre sur l’Eau de l’Union Europeenne (DCE). L’etude a permis d’identifier 26405 individus de macroinvertebres repartis en 71 especes et 33 familles au total sur les trois sites de Bama (25 familles), Boura (23 familles) et Bala (30 familles). Les polluo-sensibles ont ete decrites au lac naturel de Bala : des crustaces de l’ordre des Cladoceres et 7 familles d’insectes (Gyrinides, Pleides, Baetides Ephemerelides, Tabanides, Ceratopogonides et Canaceides). L’etude a note la presence de l’espece Hydrocanthus ferruginicollis (Noterides) particulierement dominante dans les retenues hydro-agricoles de Bama (30,52% des Coleopteres) et Boura (27,32% des Coleopteres) ; tandis qu’elle ne represente que 7,51% des Coleopteres recoltes dans le lac naturel de Bala. Cette etude considere que les polluo-sensibles et les polluo-resitants decrits respectivement a Bala et a Bama et Boura peuvent etre consideres comme de potentiels bioindicateurs des plans d’eau du bassin de la Volta. Objectif : Cette etude a pour objectif de mette en evidence l’impact de l’aquaculture en general et de la pisciculture en particulier sur la vie des communautes rurales au Burkina Faso. Methodologie et resultats : Les enquetes socio-economique dans 5 regions d’aquaculture communautaire indique que les populations sont unanimes a reconnaitre que l’aquaculture communautaire a plus contribue a exprimer la solidarite entre les villageois (48,6%), a offrir des opportunites de se ravitailler en poissons (47,38%), a ameliorer le niveau de vie au village (46,84%), enfin a donner l’occasion aux enfants d’augmenter leur bol alimentaire en proteine animale et de valoriser le poisson dans tous ses aspects esthetiques et culturels (42,74%). En outre il a reduit les inegalites economiques entre et dans les villages (0,2 %) et n’a pas du tout engendrer de nouveaux problemes. Parallelement, l’etude de la perception des memes populations sur la pisciculture integree aux autres activites agricoles, effectuee aupres de 150 familles d'exploitants des plaines irriguees, revele que 49,1 % des exploitants sont receptifs a la pisciculture en etang, 43,4 % a la rizipisciculture et 7,5 % a la pisciculture de repeuplement. Conclusion et Application des resultats : Il est conclu de ce diagnostic que l’aquaculture est une activite de developpement bien percue des populations ; de maniere specifique l’etude demontre que le mythe qui entoure les activites de pisciculture peut etre transforme en une realite economique generatrice de revenu et de proteine animale supplementaire pour les producteurs. The advent of AIDS has resulted in an increase in diseases which were previously rare. In HIV-infected subjects, histoplasmosis takes a disseminated and particularly severe form, with a fatal outcome in our country, the more so because the clinical presentation resembles tuberculosis and is generally treated as such. The authors report a case of disseminated histoplasmosis in a patient dually-reactive to HIV-1 and HIV-2, in whom the clinical presentation was fever, mediastinal lymphadenopathy and pulmonary infiltrates. The diagnosis of histoplasmosis was made at autopsy.

Introduction La prevalence de l'hypertension arterielle (HTA) est plus elevee dans la population diabetique que dans la population generale. Cette association HTA-diabete a des effets deleteres, entrainant un developpement plus precoce et plus severe des complications degeneratives du diabete. Objectif Analyser le profil epidemiologique et la repartition des complications degeneratives retrouvees chez les diabetiques hypertendus suivis au Service d'Endocrinologie Diabetologie du CHU de Yopougon en Cote d'Ivoire Materiels et methodes Il s'agit d'une etude retrospective portant sur l'analyse de 230 dossiers de patients diabetiques suivis en consultation d'Endocrinologie-Diabetologie a Abidjan entre 1986 et 2012. Resultats L'âge moyen des patients etait de 50,44±12,71 (20–85) ans, repartis en 91,8 % de diabete de type 2 ; 6,5 % de diabete de type 1 et 1,7 % de diabete secondaire, avec un sex-ratio de 1,44. La duree moyenne d'evolution du diabete etait de 20,88±6,68 (0–47) ans et celle du suivi de 5,81±5,67 (0–30) ans. Le niveau moyen de pression arterielle pour l'ensemble de la population etait de 152,18±24,55 (80–230) mmHg pour la systolique et 104,12±95,79 (65–91) mmHg pour la diastolique, le BMI moyen etait de 24,90±4,93kg/m 2 . Les complications retrouvees etaient : la polyneuropathie 59,7 % (138/231), la retinopathie diabetique 20,3 % (47/231), la dysfonction erectile 14,7 % (34/231), la cardiopathie ischemique 12,1 % (28/231), la nephropathie diabetique 9,5 % (22/231), le pied diabetique 7,7 % (18/231) et l'accident ischemique cerebral 1,7 % (4/231). Conclusion L'equilibre du diabete associe a la prise en charge des autres facteurs de risque cardiovasculaire tels que l'hypertension arterielle permet de limiter les complications degeneratives du diabete et ses consequences.

Introduction La prevalence de l’inobservance est elevee dans les maladies chroniques, ou les soins sont le fait des patients, devenus des « gestionnaires » de leur etat de sante. L’inobservance therapeutique reste une cause majeure d’echec therapeutique et remet en question l’efficacite reelle des soins. Objectif : Evaluer l’observance therapeutique des patients diabetiques de type 2 traites par antidiabetiques oraux. Materiels et methodes Notre etude etait de type descriptif et s’est deroulee de Janvier 2009 a Decembre 2009 avec un effectif de 100 patients. Etaient inclus tous les diabetiques de type 2 traites par antidiabetique oral, suivis au CHU Gabriel Toure de Bamako. Resultats La moyenne d’âge etait de 65 ans avec un sexe ratio de 0,28. L’observance therapeutique etait bonne chez 45 % de nos patients et mauvaise chez 55 %. Lorsque la duree du traitement n’excedait pas 2 ans l’observance therapeutique etait respectee par 69 % des patients, par contre lorsque le patient etait sous antidiabetique oral depuis plus de 6 ans, celle-ci etait respectee dans seulement 35 % des cas. Lorsque le nombre de prise journaliere de l’antidiabetique etait egal a trois, 65 % des patients etaient inobservants, par contre lorsque l’antidiabetique etait en mono prise l’observance etait bonne chez 70 % des patients. Les autres causes d’inobservance etaient : le manque de moyens financiers (57 %), la lassitude du patient (23 %), l’inaccessibilite au medicament (20 %). Conclusion Dans ce travail princeps, les difficultes financieres ont ete la cause la plus frequemment associee a l’inobservance therapeutique au Mali, d’ou la necessite d’une couverture sociale pour ameliorer la prise en charge des patients diabetiques.

As Bamako, Mali, grows, a man and two partners have set up a car wash on Independence Boulevard across from the Labor Exchange. The initial 150,000 CFA investment was to install a water faucet, build a large concrete slab, bring electricity to the location, and buy a small supply of tools. Since Bamako is expanding, vehicles are no longer a luxury and mud and dust are all over. After covering expenses and paying the communal tax, the men have enough to live on. The charge is 250 CFA for a simple wash and 1000 CFA for a wash including the engine. The owner envisions putting in a vehicle accessory store next door. Also on Independence Boulevard at the level of the ex-seat of National Union of Women in Mali, 2-3 teams of about 10 youth each carry buckets of water from the public fountain to clean cars in unregulated parking spaces for 4-6 CFAs. They operate without authorization and pay no taxes. But in this time where social unraveling is the force of the law, how does one prohibit this activity without provoking a protest movement which no one knows how to stop? The youth are not delinquents but rural and urban youth with uncertain schooling. To invest heavily in an activity which will not pay off, station managers prefer to align themselves towards the most sensible solutions. Thus, with the Malinian Society of Guardian Service and Care (SOMAGES) acting as an intermediary, gas stations have hired youth to do maintenance work. This gives youth stable employment, registration with the National Social Security Institute, and benefits of a monthly salary. Operators have hired hostesses that, they hope, will improve the reception quality at their stations. The hostesses inquire about the wanted service and clean the windshield. Their feminine presence contributes to customer loyalty.

Pythium aphanidermatum was consistently transmitted to healthy potted cucumber (Cucumis sativus «County Fair») plants grown in an ebb-and-flow subirrigation system when the irrigation solution was artificially infested. In contrast, transfer of the fungus was low and sporadic when some plants growing in infested medium were placed in the system among plants growing in noninfested medium. Metalaxyl was more effective than etridiazole in controlling Pythium infection when granular formulations were incorporated into the growing medium

Sanogo, S., Yang, X. B., and Scherm, H. 2000. Effects of herbicides on Fusarium solani f. sp. glycines and development of sudden death syndrome in glyphosate-tolerant soybean. Phytopathology 90:57-66. Sudden death syndrome of soybean, caused by Fusarium solani f. sp. glycines, is a disease of increasing economic importance in the United States. Although the ecology of sudden death syndrome has been extensively studied in relation to crop management practices such as tillage, irrigation, and cultivar selection, there is no information on the effects of herbicides on this disease. Three herbicides (lactofen, glyphosate, and imazethapyr) commonly used in soybean were evaluated for their effects on the phenology of F. solani f. sp. glycines and the development of sudden death syndrome in four soybean cultivars varying in resistance to the disease and in tolerance to glyphosate. Conidial germination, mycelial growth, and sporulation in vitro were reduced by glyphosate and lactofen. In growth-chamber and greenhouse experiments, there was a significant increase in disease severity and frequency of isolation of F. solani f. sp. glycines from roots of all cultivars after application of imazethapyr or glyphosate compared with the control treatment (no herbicide applied). Conversely, disease severity and isolation frequency of F. solani f. sp. glycines decreased after application of lactofen. Across all herbicide treatments, severity of sudden death syndrome and isolation frequency were lower in disease-resistant than in susceptible cultivars. Results suggest that glyphosate-tolerant and -nontolerant cu ltivars respond similarly to infection by F. solani f. sp. glycines after herbicide application.
