3. Obesity Prevention in Young Schoolchildren: Results of a Pilot Study

Abstract

Background: Overweight and obesity occur in 17% of children in the United States. Complications of excess weight in Americans cause 216,000 to 300,000 deaths yearly and cost $147 billion. Methods: A convenience sample of 14 intervention and 15 control schools in the Catholic Diocese of Pittsburgh was used. A program to improve lifestyle (Values Initiative Teaching About Lifestyle ["VITAL[R]"]), was implemented in young children to encourage healthy eating and appropriate physical activity. Students had annual evaluations of height and weight over a 2-year period, and teachers participating in "VITAL" completed questionnaires regarding the program. Changes in age- and sex-adjusted body mass index (BMI) percentiles in control and intervention groups were compared using linear mixed models regression. Results: "VITAL" was highly rated by teachers and was popular with children. Over the 2-year period, adjusted mean BMI percentiles declined from 66.1 to 65.0 in control children and from 62.8 to 58.9 in intervention children. The rates of change in the 2 groups were significantly different (p = 0.015). Conclusion: "VITAL" improves lifestyle of young schoolchildren, is inexpensive, easy to implement, and should be expanded to improve health and reduce healthcare's financial burden. 9. Reducing the public health burden from elevated blood pressure levels in the United States by lowering intake of dietary sodium

Abstract

Elevated blood pressure levels are a major cause of heart disease and stroke. Healthy People 2010 established objectives to reduce mortality from these diseases by 20% and to reduce the major causal factors associated with these elevated levels, such as excess sodium intake. The American public consumes far more sodium than is needed, most of which is added by food manufacturers and restaurants. In November 2002, the American Public Health Association adopted a policy resolution calling for a 50% reduction in sodium in the nation's food supply over the next 10 years. Such a reduction would greatly enhance the chances of attaining the Healthy People 2010 objectives and would save at least 150000 lives annually. This issue warrants public health intervention.

Abstract

Hypertensive heart disease, or heart disease caused by hypertension, is the result of increased arterial pressure and increased resistance of peripheral blood vessels. An increased workload on the heart can cause left ventricular hypertrophy, or enlargement of the left ventricle of the heart, and cardiac disorders such as congestive heart failure, irregular heart beat and ischemic heart disease. Older individuals and those with blood pressure over 160/95 have a higher risk of developing left ventricular hypertrophy than others. Left ventricular hypertrophy is caused by an increase in heart cell size and structural changes. Activation of genes that regulate cell division also occurs. Echocardiography is the most accurate method for diagnosing patients with left ventricular hypertrophy. Patients with left ventricular hypertrophy are treated with antihypertensive drugs.

One of the most successful public health programs in the past century provides an example of what can be accomplished when the government, the private sector, academia, and community organizations work together. The results of 4 decades of activities of the National High Blood Pressure Education Program (NHBPEP) can be measured in several ways. The publics’ awareness, treatment, and control have increased remarkably. Hypertension is the primary reason adults visit physicians. Age‐adjusted mortality for heart disease and stroke has declined by 70% and 80%, respectively, since the beginning of the program. The decline in heart and stroke deaths is seen in both sexes and blacks and whites, and is particularly evident in people who reside in the southeastern portion of the United States, which once had the highest mortality rates of stroke in the United States. This dramatic decrease in strokes and heart disease has occurred despite the substantial increase in obesity and diabetes in the United States.
