1. Solid-state 13C-NMR spectroscopic determination of side-chain mobilities in zirconium-based metal–organic frameworks

Abstract

Porous interpenetrated zirconium–organic frameworks (PIZOFs) are a class of Zr-based metal–organic frameworks (MOFs) which are composed of long, rod-like dicarboxylate linkers and Zr6O4(OH)4(O2C)12 nodes. Long oligoethylene glycol or aliphatic side chains are covalently attached to the linker molecules in the cases of PIZOF-10 and PIZOF-11, respectively. These side chains are supposedly highly mobile, thus mimicking a solvent environment. It is anticipated that such MOFs could be used as a solid catalyst – the MOF – with pore systems showing properties similar to a liquid reaction medium. To quantify the side-chain mobility, here we have applied different 1D and 2D NMR solid-state spectroscopic techniques like cross-polarization (CP) and dipolar-coupling chemical-shift correlation (DIPSHIFT) studies. The rather high 1H-13C CP efficiency observed for the CH2 groups of the side chains indicates that the long side chains are unexpectedly immobile or at least that their motions are strongly anisotropic. More detailed information about the mobility of the side chains was then obtained from DIPSHIFT experiments. Analytical expressions for elaborate data analysis are derived. These expressions are used to correlate order parameters and to slow motional rates with signals in indirect spectral dimensions, thus enabling the quantification of order parameters for the CH2 groups. The ends of the chains are rather mobile, whereas the carbon atoms close to the linker are more spatially restricted in mobility.

3. [Lay version of the 2018 EULAR recommendations for physical activity in people with inflammatory arthritis and osteoarthritis : Translation into German and linguistic validation in German-speaking countries with people affected]

Abstract

Physical activity and exercise are beneficial for people with rheumatic diseases; however, recommendations for the management of rheumatoid arthritis (RA), spondyloarthritis (SpA) and hip- and knee osteoarthritis (HOA/KOA) are usually unspecific with respect to mode and dose of exercise. This is why the 2018 EULAR recommendations for physical activity in people with inflammatory arthritis and osteoarthritis were formulated. The recommendations consist of 4 overarching principles and 10 recommendations. These were also published as a lay version in the English language.Translation of the lay version into German and its linguistic validation in Austria, Germany and Switzerland.A professional translation was reviewed by the authors, including people with, RA, SpA, HOA/KOA from the three German-speaking countries, which provided a prefinal lay version. Subsequently, eight interviews with people with RA, SpA, HOA/KOA were conducted in each country to evaluate understandability, wording, completeness and feasibility of the prefinal lay version. Finally, the authors, i.e. those with RA, SpA, and osteoarthritis, anonymously rated their agreement to the final lay version on a 0-10 scale.The professional translation was substantially revised by the authors and based on the interviews. Formulations were adapted to increase readability and understandability and specify statements. Comments that would have changed content or structure were not considered. Average agreement with the particular recommendations was between 10 (SD 0) and 7.6 (SD 1.67).For people with RA/SpA/HOA/KOA the EULAR physical activity recommendations should be available in their mother language. The final German lay version is valid and accepted across all three German-speaking countries. Thus, the physical activity recommendations can be provided to people with rheumatic diseases in an understandable and feasible way.HINTERGRUND: Körperliche Aktivität und spezifisches Training haben großen gesundheitlichen Nutzen. Empfehlungen zum Management von rheumatoider Arthritis (RA), Spondyloarthritis (SpA) sowie Hüft- und Kniegelenkarthose (HKA) sind bisher in Bezug auf Art und Dosierung aber unspezifisch. Darum wurden die 2018 EULAR Empfehlungen zu körperlicher Aktivität für Menschen mit entzündlich-rheumatischen und degenerativen Erkrankungen formuliert. Sie bestehen aus 4 übergeordneten Prinzipien und 10 Empfehlungen. Die EULAR Bewegungsempfehlungen wurden auch als laienverständliche Version in englischer Sprache publiziert. ZIEL: Übersetzung der laienverständlichen Version ins Deutsche und sprachliche Validierung in Deutschland, Österreich und der Schweiz.Eine professionelle Übersetzung ins Deutsche wurde von den Autor*innen einschließlich Personen mit RA, SpA und HKA zu einer präfinalen Version bearbeitet. Anschließend wurden in den 3 Ländern je 8 Interviews mit Personen mit RA, SpA und HKA durchgeführt, um die Verständlichkeit, Wortwahl, Vollständigkeit und Umsetzbarkeit zu prüfen. Die Patientenvertreter*innen der Autor*innengruppe bewerteten anonym ihre Zustimmung zur finalen Version auf einer Skala von 0 bis 10.Die professionelle Übersetzung wurde von den Autor*innen und auf Grundlage der Interviews substanziell überarbeitet. Dabei wurden Formulierungen angepasst, um Lesbarkeit und Verständlichkeit zu verbessern und Aussagen zu präzisieren. Inhalt und Struktur des Originaltextes wurden dabei nicht verändert. Die Zustimmung zu den einzelnen Empfehlungen lag zwischen 10 (SD 0) und 7,6 (SD 1,67).Für Menschen mit RA/SpA/HKA sollten die EULAR Bewegungsempfehlungen in deren Muttersprache vorliegen. Die laienverständliche deutsche Version ist valide und wurde in allen 3 Ländern gut akzeptiert. Damit können die EULAR Bewegungsempfehlungen verständlich und praktikabel vermittelt werden.

Abstract

The physics motivation, detector design, triggers, calibration, alignment, simulation, and overall performance of the very forward CASTOR calorimeter of the CMS experiment are reviewed. The CASTOR Cherenkov sampling calorimeter is located very close to the LHC beam line, at a radial distance of about 1 cm from the beam pipe, and at 14.4 m from the CMS interaction point, covering the pseudorapidity range of -6.6 < eta < - 5.2. It was designed to withstand high ambient radiation and strong magnetic fields. The performance of the detector in measurements of forward energy density, jets, and processes characterized by rapidity gaps, is reviewed using data collected in proton and nuclear collisions at the LHC.

Abstract

Erworben im Rahmen der Schweizer Nationallizenzen (http://www.nationallizenzen.ch), Hintergrund: Körperliche Aktivität und spezifisches Training haben großen gesundheitlichen Nutzen. Empfehlungen zum Management von rheumatoider Arthritis (RA), Spondyloarthritis (SpA) sowie Hüft- und Kniegelenkarthose (HKA) sind bisher in Bezug auf Art und Dosierung aber unspezifisch. Darum wurden die 2018 EULAR Empfehlungen zu körperlicher Aktivität für Menschen mit entzündlich-rheumatischen und degenerativen Erkrankungen formuliert. Sie bestehen aus 4 übergeordneten Prinzipien und 10 Empfehlungen. Die EULAR Bewegungsempfehlungen wurden auch als laienverständliche Version in englischer Sprache publiziert. Ziel: Übersetzung der laienverständlichen Version ins Deutsche und sprachliche Validierung in Deutschland, Österreich und der Schweiz. Methoden: Eine professionelle Übersetzung ins Deutsche wurde von den Autor*innen einschließlich Personen mit RA, SpA und HKA zu einer präfinalen Version bearbeitet. Anschließend wurden in den 3 Ländern je 8 Interviews mit Personen mit RA, SpA und HKA durchgeführt, um die Verständlichkeit, Wortwahl, Vollständigkeit und Umsetzbarkeit zu prüfen. Die Patientenvertreter*innen der Autor*innengruppe bewerteten anonym ihre Zustimmung zur finalen Version auf einer Skala von 0 bis 10. Ergebnisse: Die professionelle Übersetzung wurde von den Autor*innen und auf Grundlage der Interviews substanziell überarbeitet. Dabei wurden Formulierungen angepasst, um Lesbarkeit und Verständlichkeit zu verbessern und Aussagen zu präzisieren. Inhalt und Struktur des Originaltextes wurden dabei nicht verändert. Die Zustimmung zu den einzelnen Empfehlungen lag zwischen 10 (SD 0) und 7,6 (SD 1,67). Diskussion: Für Menschen mit RA/SpA/HKA sollten die EULAR Bewegungsempfehlungen in deren Muttersprache vorliegen. Die laienverständliche deutsche Version ist valide und wurde in allen 3 Ländern gut akzeptiert. Damit können die EULAR Bewegungsempfehlungen verständlich und praktikabel vermittelt werden. Background: Physical activity and exercise are beneficial for people with rheumatic diseases; however, recommendations for the management of rheumatoid arthritis (RA), spondyloarthritis (SpA) and hip- and knee osteoarthritis (HOA/KOA) are usually unspecific with respect to mode and dose of exercise. This is why the 2018 EULAR recommendations for physical activity in people with inflammatory arthritis and osteoarthritis were formulated. The recommendations consist of 4 overarching principles and 10 recommendations. These were also published as a lay version in the English language. Aim: Translation of the lay version into German and its linguistic validation in Austria, Germany and Switzerland. Methods: A professional translation was reviewed by the authors, including people with, RA, SpA, HOA/KOA from the three German-speaking countries, which provided a prefinal lay version. Subsequently, eight interviews with people with RA, SpA, HOA/KOA were conducted in each country to evaluate understandability, wording, completeness and feasibility of the prefinal lay version. Finally, the authors, i.e. those with RA, SpA, and osteoarthritis, anonymously rated their agreement to the final lay version on a 0–10 scale. Results: The professional translation was substantially revised by the authors and based on the interviews. Formulations were adapted to increase readability and understandability and specify statements. Comments that would have changed content or structure were not considered. Average agreement with the particular recommendations was between 10 (SD 0) and 7.6 (SD 1.67). Discussion: For people with RA/SpA/HOA/KOA the EULAR physical activity recommendations should be available in their mother language. The final German lay version is valid and accepted across all three German-speaking countries. Thus, the physical activity recommendations can be provided to people with rheumatic diseases in an understandable and feasible way

Abstract

A measurement is reported of differential top quark pair (t (t) over bar) production cross sections, where top quarks are produced at large transverse momenta. The data collected with the CMS detector at the LHC are from pp collisions at a center-of-mass energy of 13 TeV corresponding to an integrated luminosity of 35.9 fb(-1). The measurement uses events where at least one top quark decays as t -> Wb -> qq(-1)b and is reconstructed as a large-radius jet with transverse momentum in excess of 400 GeV. The second top quark is required to decay either in a similar way or leptonically, as inferred from a reconstructed electron or muon, a bottom quark jet, and missing transverse momentum due to the undetected neutrino. The cross section is extracted as a function of kinematic variables of individual top quarks or of the t (t) over bar system. The results are presented at the particle level, within a region of phase space close to that of the experimental acceptance, and at the parton level and are compared to various theoretical models. In both decay channels, the observed absolute cross sections are significantly lower than the predictions from theory, while the normalized differential measurements are well described.

9. ‘Distancers’ and ‘non-distancers’? The potential social-psychological impact of moralising COVID-19 mitigating practices on sustained behaviour change

Abstract

COVID-19 mitigating practices such as “hand-washing”, “social distancing” or “social isolating” are constructed as ‘moral imperatives’, required to avert harm to oneself and others. Adherence to COVID-19 mitigating practices is presently high among the general public, and stringent lockdown measures supported by legal and policy intervention has facilitated this. In the coming months, however, as rules are being relaxed and individuals become less strict, and thus the ambiguity in policy increases, the maintenance of recommended social distancing norms will rely on more informal social-interactional processes. We argue that the moralisation of these practices, twinned with relaxations of policy, may likely cause interactional tension between those individuals who do vs. those who do not uphold social distancing in the coming months: i.e., derogation of those who adhere strictly to COVID-19 mitigating practices and group polarisation between ‘distancers’ and ‘non-distancers’. In this paper we explore how and why these processes might come to pass, their impact on an overall societal response to COVID-19, and the need to factor such processes into decisions regarding how to lift restrictions.

Abstract

A search for the production of a narrow-width resonance decaying into a pair of Higgs bosons decaying into the bbZZ channel is presented. The analysis is based on data collected with the CMS detector during 2016, in proton-proton collisions at the LHC, corresponding to an integrated luminosity of 35.9 fb(-1). The final states considered are the ones where one of the Z bosons decays into a pair of muons or electrons, and the other Z boson decays to either a pair of quarks or a pair of neutrinos. Upper limits at 95% confidence level are placed on the production of narrow-width spin-0 or spin-2 particles decaying to a pair of Higgs bosons, in models with and without an extended Higgs sector. For a resonance mass range between 260 and 1000 GeV, limits on the production cross section times branching fraction of a spin-0 and spin-2 resonance range from 0.1 to 5.0 pb and 0.1 to 3.6 pb, respectively. These results set limits in parameter space in bulk Randall-Sundrum radion, Kaluza-Klein excitation of the graviton, and next-to-minimal two-Higgs doublet models (N2HDMs). For specific choices of parameters the N2HDM can be excluded in a mass range between 360 and 620 GeV for a resonance decaying to two Higgs bosons. This is the first search for Higgs boson resonant pair production in the bbZZ channel.

IntroductionVideo-recorded disclosures of two siblings emerged describing ritual violence activities in a Hampstead “cult” including human sacrifice murder. The UK court processes judged that the mother “coached” the children to make false allegations. In parallel, Marie Black, Jason Adams and Michael Roger were found guilty of child sexual abuse purely based on historical accounts of five children.ObjectivesThe presentation outlines two cases that illustrate the “unbelievable” nature of organized child abuse and what aspects make court rulings “unsafe”.AimsThe presentation aims to compare the Hampstead where the videos were watched by 4 million individuals world-wide with the little that is known about the “Norwich Three” case due to court reporting restrictions.MethodsMaterials posted on websites, books and videos were reviewed alongside discussions with whistle-blowers, relatives and friends of people involved.ResultsThe disclosures in the Hampstead case were very extreme but similar accounts can be found in books by Sara Scott (UK) and De Camp (US). Those accused of sexual abuse claim that the children have been coached by their mother to make these allegations–the view that the judge adopted. In the Norfolk case, 7 of the 10 defendants were cleared of child sexual abuse allegations but three individuals were found guilty. Infolge der starken Zunahme padiatrischen Wissens, das die Kapazitaten der Allgemeinpadiater ubersteigt, etablierten sich national und international Subspezialbereiche zwecks zeitgemaser und bedarfsgerechter Diagnostik und Therapie von Kindern und Jugendlichen mit besonderen, aufwendigen und seltenen Krankheiten. Um allen einschlagigen Patienten unseres Landes eine angemessene medizinische Betreuung zukommen lassen zu konnen, erscheint ein Optimierungsplan mit patientengerechter Schwerpunktsetzung von Spezialabteilungen an verschiedenen Zentren als zielfuhrend. Nach dem Vorbild der Schweizer Padiatrie, aber auch einzelner Spezialbereiche in Osterreich erscheinen die spezialisierten Arbeitsgruppen der Osterreichischen Gesellschaft fur Kinder- und Jugendheilkunde als pradestiniert, bedarfsgerechte, flachendeckende Programme zu erstellen, an welchen Zentren derartige Schwerpunkte mit zeitgemaser Infrastruktur und Forschungsmoglichkeiten mit internationaler Kooperation zu etablieren waren.

Abstract

The feasibility of electrically conductive paper as a low-cost and eco-friendly alternative to copper is presented for application as an antenna conductor in UHF radio-frequency identification (RFID) systems. The conductivity of the paper is determined to be 50 S/m and the effect of such a low conductivity on the radiation performance of a half-wavelength dipole is investigated through simulation and experimental measurements. The radiation efficiency of a dipole cut from a 0.4 mm thick conductive paper sheet is measured to be 5.0%, which translates to an RFID tag read range of 22% of that obtained using copper. This performance is deemed unacceptable for most antenna applications. However, through simulation a conductivity of 500 S/m is identified as a reasonable target for the developers of conductive paper as the current distribution and terminal properties of the resulting antenna become similar to those observed when using copper. The radiation efficiency is projected at 56 % in this case, resulting in an RFID read range of 73% of that obtained using copper.

Review of the sparse literature and media coverage was used to ascertain themes.ResultsColin Batley, a self-styled Satanist high priest, was sentenced for child sexual abuse alongside 4 others. His son Damien had died through aspiration when filming himself in a “sexual stimulation game”.Ian Watkins, disgraced singer of “The Lost Prophets”, was sentenced for abusing two babies provided by two “super-fan” mums. Peaches Geldorf tweeted their names, got a tattoo of O.T.O, a Satanist “kinky sex” group and was found dead a year later.Albert and Carole Hickman were sentenced for sexually abusing an 11-year-old including “sucking blood”.Matthew Williams cannibalised his victim before being killed with a police taser.Conclusions“Hanging” and “Over-dosing” could be the result of suicide or of “being suicided”. The abuse crimes of all three Satanists convicted in the last 15 years in UK courts occurred in South Wales. Abstract

Uniform treatment based on the therapeutic approach of the 1st and 2nd US National Wilms' Tumor Study was decided on in March 1976 by paediatricians, surgeons, urologists and radiotherapists in Austria. Wilms' tumour was diagnosed in 34 children between 1 january 1976 an 29 february 1980 (stage I: n = 11, stage II: n = 8, stage III: n = 8, stage IV: n = 7). Parents of two children refused treatments; both children have since died of metastases. Of the remaining 32 children 29 (90.6%) are alive, 10 for more than 4, 15 for more than 3 and 19 for more than 2 years after diagnosis. 21 children are without need of treatment. Three children have died, one due to postoperative complications, one due to haemorrhagic chickenpox, but free of tumour, and one after insufficient treatment. Two of the five children with a recurrence between 2 1/4 to 15 months after diagnosis had been treated inadequately in the initial phase. The tumour free survival rate in 74.2%. Two children with early occurring or recurrent lung metastases have survived for 53 1/2 and 54 months up to now.

Abstract

Esophageal atresia with or without tracheoesophageal fistula (EA/TEF) and anorectal malformations (ARM) represent the severe ends of the fore- and hindgut malformation spectra. Previous research suggests that environmental factors are implicated in their etiology. These risk factors might indicate the influence of specific etiological mechanisms on distinct developmental processes (e.g. fore- vs. hindgut malformation). The present study compared environmental factors in patients with isolated EA/TEF, isolated ARM, and the combined phenotype during the periconceptional period and the first trimester of pregnancy in order to investigate the hypothesis that fore- and hindgut malformations involve differing environmental factors. Patients with isolated EA/TEF (n = 98), isolated ARM (n = 123), and the combined phenotype (n = 42) were included. Families were recruited within the context of two German multicenter studies of the genetic and environmental causes of EA/TEF (great consortium) and ARM (CURE-Net). Exposures of interest were ascertained using an epidemiological questionnaire. Chi-square, Fisher's exact, and Mann-Whitney U-tests were used to assess differences between the three phenotypes. Newborns with isolated EA/TEF and the combined phenotype had significantly lower birth weights than newborns with isolated ARM (P = 0.001 and P0.0001, respectively). Mothers of isolated EA/TEF consumed more alcohol periconceptional (80%) than mothers of isolated ARM or the combined phenotype (each 67%). Parental smoking (P = 0.003) and artificial reproductive techniques (P = 0.03) were associated with isolated ARM. Unexpectedly, maternal periconceptional multivitamin supplementation was most frequent among patients with the most severe form of disorder, i.e. the combined phenotype (19%). Significant differences in birth weight were apparent between the three phenotype groups. This might be attributable to the limited ability of EA/TEF fetuses to swallow amniotic fluid, thus depriving them of its nutritive properties. Furthermore, the present data suggest that fore- and hindgut malformations involve differing environmental factors. Maternal periconceptional multivitamin supplementation was highest among patients with the combined phenotype. This latter finding is contrary to expectation, and warrants further analysis in large prospective epidemiological studies.

Fast alle Energieszenarien stützen sich auf Trends, die auf ein enormes Wachstum des Energiebedarfs in den nächsten Jahrzehnten hinauslaufen. Auf internationalen Konferenzen beschäftigt man sich indes mit dem Gegenteil: einer massiven Verringerung der Treibhausgasemissionen, vor allem der durch den Verbrauch von Energie verursachten CO2-Emissionen. Experten verweisen auch auf das politische Risiko der Abhängigkeit vom Erdöl und die Erschöpflichkeit von Ressourcen. Wie soll diese Kluft überbrückt werden? Wie lässt sich das bestehende Energiesystem nachhaltiger gestalten? Die Hoffnungen richten sich vor allem auf den technischen Fortschritt und Innovationen. Bislang liegen jedoch keine konkreten Vorschläge vor, inwieweit Innovationen tatsächlich dazu beitragen können, einen ständig wachsenden Energieverbrauch mit den Limitierungen im Hinblick auf die Ressourcenverfügbarkeit und die Umwelt sowie den strukturellen Anforderungen an ein Energiesystem in Einklang zu bringen. Die vorliegende Studie führt wirtschaftswissenschaftliche, juristische, naturwissenschaftliche und philosophische Kompetenzen im Hinblick auf derartige Vorschläge zusammen. Aufbauend auf einer soliden Rekonstruktion der mitunter verwirrenden Vielfalt der Begriffe werden Beurteilungen des bestehenden Energiesystems vorgelegt und Nachhaltigkeitspotenziale aufgezeigt. Die strategisch gebündelten Handlungsempfehlungen berücksichtigen die vielfältigen Zielkonflikte umwelt- und energiepolitischen Handelns und werden durch Implementierungsvorschläge ergänzt.

Abstract

In der Padiatrie besteht der Bedarf an besserer wissenschaftlich fundierter medizinischer Praxis auf der Grundlage qualitativ hochwertiger Forschung uber Wirksamkeit und Sicherheit der medizinischen Behandlung von Kindern. Um die „besten Interessen des Kindes“ zu wahren, muss ein Gleichgewicht bestehen zwischen den ethischen Forderungen nach klinischer Forschung und gleichzeitiger Verhinderung jeglichen Schadens. Somit mussen die Grundsatze der „guten klinischen Praxis“, die in der Deklaration von Helsinki und in den Richtlinien der ICH, der EU-Direktive und anderen einschlagigen Dokumenten festgelegt wurden, in der padiatrischen Forschung eingehalten werden. Evidente Unterschiede in Physiologie, Pathologie, Pharmakokinetik und -dynamik zwischen verschiedenen Altersgruppen und zwischen Kindern und Erwachsenen verlangen einwandfrei geplante und durchgefuhrte Studien, die die speziellen somatischen, emotionalen und mentalen Bedurfnisse der Kinder berucksichtigen. IntroductionIn 2012, forensic psychology Professor Jane Ireland published initial research claiming that two third of psychological assessment reports sampled from UK family courts were ‘poor’ or ‘very poor’. ‘Fitness to practice’ concerns were raised by vested interest and dismissed after a 1-week hearing – four years later.ObjectivesThe presentation outlines the nature of various UK institutions, such as family courts, HCPC and GMC as well as their practices which raise questions about their fitness to regulate.AimsDelegates will start to learn how institutions that purport to serve public interest yet can be easily exploited by vested interests.MethodsCase studies are used to illustrate how extremely serious concerns were ignored but persecution concerns upheld.ResultsIn one case, four courts appointed experts ignored an obvious child trafficking process where a toddler was raped to cover up birth and disappearance of a newborn baby that succeeded from incestuous rape. In spite of a clinical psychologist failing to cover the two index incidents, the concerns did not meet the HCPC ‘Standard of Acceptance’. A ‘revenge concern’ was raised by vested interests. In another case, the GMC refused to investigate a psychiatrist who had lied and rather absurdly claimed that repeatedly seeking return of her children was evidence for a mother's personality disorder. Psychiatrists of renown such as David Hawkins and Colin Ross have embraced and written about energetic medicine in their practice.ObjectivesThe presentation explains how somatoform dissociations are tell-tale indications of abuse and neglect of early childhood trauma and how ‘Energetics’ therapy facilitates healing.AimsDelegates will learn to recognise somatoform dissociation symptoms, understand advances and limitations of psychometric assessment tools, appreciate energetics approaches as an adjunct to other intervention methods and gain an insight into the origins of complex trauma.MethodsTwo case studies are used to illustrate causes, impact, diagnosis and healing of complex trauma.ResultsA set of psychometric assessments helped to unravel a chilling revictimisation crime series. ‘Twice Exceptional’ characteristics were very high IQ coupled with Dyslexia, very weak auditory memory and psychic capabilities. In another case that stemmed from extreme abuse of ancient, commercial and high-tech varieties muscle testing and energetics therapy lead to a remarkable recovery.ConclusionsPsychometric and muscle testing can inform diagnosis, therapy and healing. Energetics can be used to bring about profound healing for those who have repressed severe trauma. IntroductionEmotional Agility constitutes an alternative to the DSM-5 personality trait model.ObjectiveThe presentation outlines how Emotional Agility is conceptualised and measured through self-report and multi-rater questionnaires.AimsThe paper highlights the development of a short Emotional Agility trait questionnaire and a corresponding behavioural measure with 18 items.MethodData (n = 929) from a substantial personality questionnaire with 161 questions was utilised to create a 54 question ‘short form’ that measures the Big 5 personality factor plus Need for Achievement through 18 facet scales of 3 items each. Data on the same subjects from a 50-item criterion measure was reduced to 18 items that are structurally aligned to the 18-predictor facets.ResultsPredictor reliabilities averaged 0.752 at Factor as well as Facet level. The unit weight sum of the 54 questions achieved an uncorrected validity of 0.28 (n = 929) against external ratings of effectiveness. Joint factor analysis showed good discrimination between the six factors. A ‘positive manifold’ of Emotional Agility scales emerged as expected which could be conceptualised as the opposite of the MMPI2 ‘Demoralisation Factor’.ConclusionThe Emotional Agility approach could ground mental health assessments firmly in positive approaches that use every-day terminology. Work-related personality questionnaires, in-depth ability tests and Lacter & Lehmann (2008) guidelines were deployed to differentiate complex trauma from an erroneous diagnosis based largely on MCMI-III results.ResultsThe research identified measurement issues with the MCMI-III clinical personality questionnaire that generated spurious elevations on Narcissistic, Delusional and Paranoid scales. Work-related personality questionnaires provided much more useful information showing no “personality disorder” risks at all. WAIS results confirmed an earlier “Twice Exceptional” ability pattern with very high verbal IQ (95%ile) and extraordinarily poor auditory working memory (2nd%ile) i.e. a “Dyslexia” performance pattern. Lacter & Lehmann (2008) guidelines showed that none of the 42 schizophrenia indicators applied and only 1/3 of the complex trauma indicators.ConclusionMental health professionals must remain cognizant to the chilling notion that extreme abusers may “frame” victims in order to make them “appear” schizophrenic. As MCMI-III was developed originally for those seeking therapy, its use in forensic settings with the general population should be avoided. Survivors provide chilling accounts of ritual killings in Scott (2001), Becker, Karriker, Overkamp and Rutz (2008) and Epstein, Schwartz and Schwartz (2011). In the wake of institutional abuse enquiries and the ‘unbelievable’ child abuse perpetrated by celebrities like Jimmy Saville and Ian Watkins, a ‘new reality’ is setting in that child abuse is pervasive and knows no limits. Reports of elaborate rituals with ‘mock’ human sacrifices at the highly secretive annual ‘Bohemian Grove’ summer festival point towards a pervasive interest in the occult in high society.ConclusionMental health professionals have a ‘duty of care’ towards their service users. The applicability of a low-noise scintillation detector (ScD) for the registration of electrons of several keV energy has been studied employing photomultipliers (PM) of different types and sizes. With the application of a sedimented P-47 scintillation screen, the values of the low-energy sensitivity limit and those of the light conversion coefficient were determined as about 2.7–4.7 keV and 2.8–6.6 photoelectrons/keV, respectively, for the set of PM's (Philips-Valvo XP 2020, Philips-Valvo XP 2052, Philips-Valvo XP 2972, EMI 9124a) studied. It is concluded that such scintillation detectors might be used advantageously as electron counters in the range of E > 5 keV. Applications below this kinetic energy value are also feasible when applying a floating acceleration of several kV to the ScD — a voltage much lower than the values required for Everhart-Thornley detectors.

Accurate coronary artery calcium scoring improves risk stratification in some strata of the population.We evaluated individual and combined effects of reader experience, heart rate, vessel displacement, and trajectory on computed tomography (CT) Agatston score, calcium volume, and calcium mass in a cardiac phantom model.A cardiac motion phantom was scanned with a 64-slice CT scanner with artificial electrocardiogram gating with combinations of the following: heart rates 60, 80, and 100 beat/min; vessel displacement of 1.25 and 2.5 cm; and multiple vessel trajectories of craniocaudal, right-left, anteroposterior, right coronary artery (RCA), left anterior descending, and left circumflex (LCX). Calcium quantification was done by 2 different readers with the use of 3 methods: Agatston, calcium volume, and calcium mass.Heart rate, coronary displacement, and trajectory had significant effects on all 3 techniques, with a general decrease in score as the heart rate increased. A vessel displacement of 2.5 cm decreased the Agatston score by 16% (P0.0001) and LCX motion decreased the score by 17% (P0.0001). Combined effects often resulted in larger differences; for example, a heart rate of 60 beat/min, vessel displacement of 1.25 cm, and RCA motion resulted in an Agatston score of 907, whereas with a heart rate of 100 beat/min, vessel displacement of 2.5 cm, and LCX motion the score was 604.The calcium score is affected by heart rate, vessel displacement, and trajectory.

BACKGROUND Although the existence of myocardial mechanoelectrical feedback is well established, the mechanism of arrhythmia induction by ventricular dilatation or stretch remains insufficiently defined. In particular, controversy exists when comparing the arrhythmogenic potential of chronic versus acute myocardial stretch. Also, assessment of cellular electrophysiological effects of myocardial stretch has been incomplete. METHODS AND RESULTS To evaluate the electrophysiological and arrhythmogenic effects of slow versus rapid ventricular wall stretch, we developed an isolated Langendorff-perfused rabbit heart model in which left ventricular (LV) volume can be changed by a computer-controlled servopump. Cellular electrophysiological effects and premature ventricular excitations (PVEs) and their origin produced by the volume increases were assessed by a multiple-site monophasic action potential (MAP) recording system and by volume-conducted ECGs obtained by immersing the entire preparation in a saline-filled tank. Volume was increased either gradually with slow volume ramps (0.1 ml/sec) or suddenly by volume pulses of varying pulse waveforms (three different amplitudes and five different rise velocities) applied randomly 250-350 times to each of eight hearts. Gradual LV volume loading caused gradual decreases in MAP resting and action potential amplitude, whereas rapid, transient volume pulses caused transient depolarizations. Despite similar membrane potential effects of stretch, gradual volume increases rarely (11%) produced PVEs, even with large volume loads, whereas rapid volume pulses of moderate amplitudes regularly triggered PVEs (45-100% of interventions). Logistic regression analysis showed that the probability of PVE occurrence increased independently with both the amplitude and the velocity of the volume increase, with the greatest sensitivity to stretch velocity exhibited at low and intermediate pulse amplitudes. Faster volume pulse rise velocities triggered PVEs at a lower instantaneous pulse amplitude than lower rise velocities, further corroborating the dependence of stretch-activated arrhythmias on the velocity of stretch. In contrast, an increase in the basic ventricular volume had no effect on the probability of PVE occurrence during the volume pulses. The MAP recordings demonstrated spatial variability in the extent of local depolarizations and site of PVE origin; transient depolarizations occurred, and PVEs originated most often in the posterolateral region of the left ventricle. CONCLUSIONS Membrane depolarization is caused by both gradual and rapid ventricular stretch, but PVEs are more easily elicited by rapid stretch. Regions of greater myocardial compliance that experience greater relative stretch may act as "foci" for stretch-activated arrhythmias during dynamic ventricular loading. These whole-heart data corroborate the existence of stretch-activated membrane channels in ventricular myocardium and may help explain ventricular ectopy under conditions of differential ventricular loading, as in ventricular dyskinesia, or regional muscle traction, as in mitral valve prolapse syndrome.

Abstract

Severe neurological impairment as the first symptom of acute leukaemia is a rather uncommon finding. We report the case of a 10-month-old infant who presented with acute paralysis of the lower extremities due to cord compression by an epidural tumour composed of malignant erythrocyte precursor cells. Diagnosis of erythroleukaemia (EL) was made by needle biopsy of the spinal epidural mass and confirmed by bone marrow aspiration. Antileukaemic treatment in combination with radiotherapy to the epidural tumour led to haematological remission and neurological recovery with disappearance of the mass lesion as demonstrated by MRI. However, haematological relapse occurred with death of the patient 7 months after diagnosis. This is the first reported case of EL presenting with paraparesis due to an epidural tumour. The clinical symptoms, results of cytogenetic and immunological studies and the clinical course are described.

Drug delivery is still a big challenge in the effective pharmaceutical treatment of diseases. The ultimate aim is the delivery of any active pharmaceutical ingrediant (API) in an appropriate therapeutic concentration at the right location without systemic adverse effects in a safe and reproducible manner. A promising technology is the use of implantable drug delivery systems. Here we report a new electroactive nanoporous valve for a drug delivery implant based on the electroactive polymer polypyrrole in a nanoporous membrane for a controlled continuous release of an API. The release is controlled by the opening of the nanopores of carrier substrate comprising the conducting polymer polypyrrole / sodium dodecylbenzenesulfonate (PPy/DBS) that is located in the nanopores on a gold layer, as conductor. The composition of a conducting redox-polymer (PPy) with the charged counterion DBS allows an overall volume change of this polymer depending on the redox-state of the PPy and therefore the opening or closing of the nanopores for the controlled release of an API.

Abstract

A two-dimensional position-sensitive 6 Li glass scintillation detector for neutrons has been constructed and put into operation in a 40 m long small angle neutron scattering instrument. The detector has a detection area of 60 × 60 cm, a neutron capture probability of 96% for subthermal neutrons of 8 A wavelength, a spatial resolution of 8 mm adapted to the scattering experiments, and a counting ability of 100 kHz. The Anger type camera consists of 8 × 8 photomultipliers of 3 in. diameter arranged in a square lattice with 8.4 cm lattice constant. In a coarse fine procedure a neutron capture event is addressed by determining the photomultiplier with maximum anode signal and by a division procedure involving the nine relevant signals of the 3 × 3 photomultiplier matrix centered at the maximum photomultiplier signal. The gain of each photomultiplier is kept constant in time by a gain stabilizing system. Construction, data processing and properties of the detector are described. A few small angle scattering patterns from polymer samples are presented.

Integral Conversion Electron Mossbauer Spectroscopy (ICEMS) when aimed at the study of clean surfaces in UHV often leads to unfavourable measuring geometry and consequently to long recording times. Possible means of shortening of the measuring times are analyzed. It is shown that criteria for experimental preferences may be distinctly different for different sample structures and for different kinds of electron detectors.

Abstract

APEX, the Atacama Pathfinder Experiment, has been successfully commissioned and is in operation now. This novel submillimeter telescope is located at 5107 m altitude on Llano de Chajnantor in the Chilean High Andes, on what is considered one of the world's outstanding sites for submillimeter astronomy. The primary reflector with 12 m diameter has been carefully adjusted by means of holography. Its surface smoothness of 17-18 mu m makes APEX suitable for observations up to 200 mu m, through all atmospheric submm windows accessible from the ground.

Abstract

The main ethical imperative of all paediatric actions is the demand to do everything "in the best interests of children". Relevant guidelines can be derived from the UN Declaration on the Rights of Children, whereupon every child has the fundamental right to life and dignity, and is entitled to optimal medical care. Paediatric care in general includes the responsibility to achieve the highest level of knowledge, consideration of the child-specific somatic, mental and social development, empathic and trustworthy communication with the child and parents, observance of the recommendations of the Charter of the Rights of Children in Hospital, and cooperation with experts in related professions. Good communication is based on respect for the dignity of the child as a person and on the use of child-specific language, recognizing the rights of the child to be involved in consent or assent. Good clinical practice dictates and demands high standards of practice in therapeutics, research and medical interventions involving children. Decision making in extreme situations with regard to continuation, withholding or withdrawing life supporting measures is amongst the most complex and ethically difficult tasks of a doctor. Ethical issues with regard to neglect, maltreatment, abuse and addictions involving children need scrupulous consideration. Paediatricians have a prime responsibility to promote and protect the well being of children.

48. Surge Avoidance in Gas Compression Systems

Abstract

The phenomenon of compressor surge and its prevention have drawn significant attention in the literature. An important aspect of surge avoidance lies in the design of the compressor station and, in particular, the piping upstream and downstream of the compressor. Most anti-surge systems are perfectly capable of avoiding surge during normal operating conditions. However, unplanned emergency shutdowns present a significant challenge, and surge avoidance in these cases depends to a large degree on the station layout. In this paper, data from a compressor that surged during an emergency shutdown are presented. The data are analyzed to determine the effects of surge and the rate of deceleration. A model to simulate shutdown events is developed and used to develop simpler rules that help with proper sizing of upstream and downstream piping systems, as well as the necessary control elements. The compression system is analyzed, thus verifying the model and the simplifications.

Abstract

Intussusception of the intestine is a frequent reason for intestinal obstruction in infants; the course is acute but the causes are often not recognizable. In adults intussusception is the cause for bowel obstruction in only 1% of cases. The course is subacute and in over 90% a benign or malignant process in this part of the intestine is found. We report the case of a 45-year-old male with abdominal discomfort, increasing over weeks. The preoperative ultrasonography and computed tomography of the abdomen revealed the typical finding of ileocoecal intussusception. An ileocoecal resection was performed. The histopathological workup showed a small carcinoid tumour of the terminal ileum.

Abstract

We describe the telescope concept of the VST-the VLT survey telescope-and the scientific and instrumental background which lead to the kick-off of the project for the realization of a 2.6 m telescope for wide field imaging in the optical and infrared to be placed at Paranal, on the VLT plateau within the year 2001. The project for the camera and the data handling and archiving, plus the analysis pipeline are outlined, with particular emphasis on the possible scientific surveys and stand alone projects. We will briefly review some of the scientific projects for which the VST was conceived, and its impact on the VLT science.
