Abstract

Mitochondrial diseases are a group of inherited diseases with highly varied and complex clinical presentations. Here, we report four individuals, including two siblings, affected by a progressive mitochondrial encephalopathy with biallelic variants in the cardiolipin biosynthesis gene CRLS1. Three affected individuals had a similar infantile presentation comprising progressive encephalopathy, bull’s eye maculopathy, auditory neuropathy, diabetes insipidus, autonomic instability, cardiac defects and early death. The fourth affected individual presented with chronic encephalopathy with neurodevelopmental regression, congenital nystagmus with decreased vision, sensorineural hearing loss, failure to thrive and acquired microcephaly. Using patient-derived fibroblasts, we characterized cardiolipin synthase 1 (CRLS1) dysfunction that impaired mitochondrial morphology and biogenesis, providing functional evidence that the CRLS1 variants cause mitochondrial disease. Lipid profiling in fibroblasts from two patients further confirmed the functional defect demonstrating reduced cardiolipin levels, altered acyl-chain composition and significantly increased levels of phosphatidylglycerol, the substrate of CRLS1. Proteomic profiling of patient cells and mouse Crls1 knockout cell lines identified both endoplasmic reticular and mitochondrial stress responses, and key features that distinguish between varying degrees of cardiolipin insufficiency. These findings support that deleterious variants in CRLS1 cause an autosomal recessive mitochondrial disease, presenting as a severe encephalopathy with multi-systemic involvement. Furthermore, we identify key signatures in cardiolipin and proteome profiles across various degrees of cardiolipin loss, facilitating the use of omics technologies to guide future diagnosis of mitochondrial diseases.

Abstract

Comment in Does HIV-1 virulence matter in the ART era? Lewitus E, Rolland M. Med (N Y). 2022 Apr 8;3(4):217-219. doi: 10.1016/j.medj.2022.03.003. PMID: 35590149; International audience; We discovered a highly virulent variant of subtype-B HIV-1 in the Netherlands. One hundred nine individuals with this variant had a 0.54 to 0.74 log 10 increase (i.e., a ~3.5-fold to 5.5-fold increase) in viral load compared with, and exhibited CD4 cell decline twice as fast as, 6604 individuals with other subtype-B strains. Without treatment, advanced HIV—CD4 cell counts below 350 cells per cubic millimeter, with long-term clinical consequences—is expected to be reached, on average, 9 months after diagnosis for individuals in their thirties with this variant. Age, sex, suspected mode of transmission, and place of birth for the aforementioned 109 individuals were typical for HIV-positive people in the Netherlands, which suggests that the increased virulence is attributable to the viral strain. Genetic sequence analysis suggests that this variant arose in the 1990s from de novo mutation, not recombination, with increased transmissibility and an unfamiliar molecular mechanism of virulence.

Abstract

Mitochondrial diseases are a group of inherited diseases with highly varied and complex clinical presentations. Here, we report four individuals, including two siblings, affected by a progressive mitochondrial encephalopathy with biallelic variants in the cardiolipin biosynthesis gene CRLS1. Three affected individuals had a similar infantile presentation comprising progressive encephalopathy, bull’s eye maculopathy, auditory neuropathy, diabetes insipidus, autonomic instability, cardiac defects and early death. The fourth affected individual presented with chronic encephalopathy with neurodevelopmental regression, congenital nystagmus with decreased vision, sensorineural hearing loss, failure to thrive and acquired microcephaly. Using patient-derived fibroblasts, we characterized cardiolipin synthase 1 (CRLS1) dysfunction that impaired mitochondrial morphology and biogenesis, providing functional evidence that the CRLS1 variants cause mitochondrial disease. Lipid profiling in fibroblasts from two patients further confirmed the functional defect demonstrating reduced cardiolipin levels, altered acyl-chain composition and significantly increased levels of phosphatidylglycerol, the substrate of CRLS1. Proteomic profiling of patient cells and mouse Crls1 knockout cell lines identified both endoplasmic reticular and mitochondrial stress responses, and key features that distinguish between varying degrees of cardiolipin insufficiency. These findings support that deleterious variants in CRLS1 cause an autosomal recessive mitochondrial disease, presenting as a severe encephalopathy with multi-systemic involvement. Furthermore, we identify key signatures in cardiolipin and proteome profiles across various degrees of cardiolipin loss, facilitating the use of omics technologies to guide future diagnosis of mitochondrial diseases.

Abstract

Permanent hearing loss affects 1-3 per 1000 children in Australia. Universal hearing screening of neonates has facilitated early diagnosis and access to hearing devices and early intervention services. Despite this clinical management and investigation of newly diagnosed infants is highly variable. The aim is to provide consensus recommendations for the investigation and clinical management of children with hearing loss for geneticists, pediatricians, otolaryngolo-gists and general practitioners. The Childhood Hearing Australasian Medical Professionals (CHAMP) network was established in 2016 to improve care for hearing-impaired children in Australasia. A working group of 15 members held round-table discussions, examined existing guidelines and completed literature reviews to create a set of recommendations. Members votedonthe grade and strengthofrec-ommendations using NHMRC guidelines. Recommendations are presented in three tiers: (1) First line investigations for non-syndromic hearing loss, (2) Additional investigations based on clinical presentation, and (3) Investigations to consider if tier 1 and 2 investigations are negative. In addition to detailed history taking and examination, all children with congenital hearing loss should have CMV testing, brain MRI, ophthalmology assessment and family audiograms. Children with bilateral SNHL should be offered genetic testing after adequate genetic counseling, with connexin/GJB2 testing as first-line and chromosome microarray as second-line. Where available, genomic testing (exome or deafness panel) should be considered in children with bilateral SNHL, and may reduce the need for other investigations. The role of genetic testing in unilateral loss is limited and should be guided by clinical presentation.

Abstract

The Ultraviolet and Visible Imaging and Spectrographic Imaging experiment consists of five spectrographic imagers and four imagers. These nine sensors provide spectrographic and imaging capabilities from 110 to 900 nm. The spectrographic imagers share an off-axis design in which selectable slits alternate fields of view (1.00 [degrees] X 0.10 [degrees] or 1.00 [degrees] X 0.05 [degrees]) and spectral resolutions between 0.5 and 4 nm. Image planes of the spectrographic imager have a programmable spectral dimension with 68, 136, or 272 pixels across each individual spectral band, and a programmable spatial dimension with 5, 10, 20, or 40 pixels across the 1 [degree] slit length. A scan mirror sweeps the slit through a second spatial dimension to generate a 1 [degree] X 1 [degree] spectrographic image once every 5, 10, or 20 s, depending on the scan rate. The four imagers provide narrow-field (1.28 [degrees] X 1.59 [degrees]) and wide-field (10.5 [degrees] X 13.1 [degrees]) viewing. Each imager has a six-position filter wheel that selects various spectral regimes and neutral densities. The nine sensors utilize intensified CCD detectors that have an intrascene dynamic range of [approximately] [10.sup.3] and an interscene dynamic range of [approximately] [10.sup.5]; neutral-density filters provide an additional dynamic range of [approximately] [10.sup.2-3]. The detector uses an automatic gain control that permits the sensors to adjust to scenes of varying intensity. The sensors have common boresights and can operate separately, simultaneously, or synchronously. To be launched aboard the Midcourse Space Experiment spacecraft in the mid-1990's, the ultraviolet and visible imaging and spectrographic imaging instrument will investigate a multitude of celestial, atmospheric, and point sources during its planned 4-yr life.

Abstract

Background: There is little information on the incidence of AIDS-defining events which have been reported in the literature to be associated with immune reconstitution inflammatory syndrome (IRIS) after combined antiretroviral therapy (cART) initiation. These events include tuberculosis, mycobacterium avium complex (MAC), cytomegalovirus (CMV) retinitis, progressive multifocal leukoencephalopathy (PML), herpes simplex virus (HSV), Kaposi sarcoma, non-Hodgkin lymphoma (NHL), cryptococcosis and candidiasis.Methods: We identified individuals in the HIV-CAUSAL Collaboration, which includes data from six European countries and the US, who were HIV-positive between 1996 and 2013, antiretroviral therapy naive, aged at least 18 years, hadCD4+ cell count and HIV-RNA measurements and had been AIDS-free for at least 1 month between those measurements and the start of follow-up. For each AIDS-defining event, we estimated the hazard ratio for no cART versus less than 3 and at least 3 months since cART initiation, adjusting for time-varying CD4+ cell count and HIV-RNA via inverse probability weighting.Results: Out of 96 562 eligible individuals (78% men) with median (interquantile range) follow-up of 31 [13,65] months, 55 144 initiated cART. The number of cases varied between 898 for tuberculosis and 113 for PML. Compared with non-cART initiation, the hazard ratio (95% confidence intervals) up to 3 months after cART initiation were 1.21 (0.90-1.63) for tuberculosis, 2.61 (1.05-6.49) for MAC, 1.17 (0.34-4.08) for CMV retinitis, 1.18 (0.62-2.26) for PML, 1.21 (0.83-1.75) for HSV, 1.18 (0.87-1.58) for Kaposi sarcoma, 1.56 (0.82-2.95) for NHL, 1.11 (0.56-2.18) for cryptococcosis and 0.77 (0.40-1.49) for candidiasis.Conclusion: With the potential exception of mycobacterial infections, unmasking IRIS does not appear to be a common complication of cART initiation in high-income countries.

Abstract

Background: There is little information on the incidence of AIDS-defining events which have been reported in the literature to be associated with immune reconstitution inflammatory syndrome (IRIS) after combined antiretroviral therapy (cART) initiation. These events include tuberculosis, mycobacterium avium complex (MAC), cytomegalovirus (CMV) retinitis, progressive multifocal leukoencephalopathy (PML), herpes simplex virus (HSV), Kaposi sarcoma, non-Hodgkin lymphoma (NHL), cryptococcosis and candidiasis. Methods: We identified individuals in the HIV-CAUSAL Collaboration, which includes data from six European countries and the US, who were HIV-positive between 1996 and 2013, antiretroviral therapy naive, aged at least 18 years, hadCD4+ cell count and HIV-RNA measurements and had been AIDS-free for at least 1 month between those measurements and the start of follow-up. For each AIDS-defining event, we estimated the hazard ratio for no cART versus less than 3 and at least 3 months since cART initiation, adjusting for time-varying CD4+ cell count and HIV-RNA via inverse probability weighting. Results: Out of 96 562 eligible individuals (78% men) with median (interquantile range) follow-up of 31 [13,65] months, 55 144 initiated cART. The number of cases varied between 898 for tuberculosis and 113 for PML. Compared with non-cART initiation, the hazard ratio (95% confidence intervals) up to 3 months after cART initiation were 1.21 (0.90-1.63) for tuberculosis, 2.61 (1.05-6.49) for MAC, 1.17 (0.34-4.08) for CMV retinitis, 1.18 (0.62-2.26) for PML, 1.21 (0.83-1.75) for HSV, 1.18 (0.87-1.58) for Kaposi sarcoma, 1.56 (0.82-2.95) for NHL, 1.11 (0.56-2.18) for cryptococcosis and 0.77 (0.40-1.49) for candidiasis. Conclusion: With the potential exception of mycobacterial infections, unmasking IRIS does not appear to be a common complication of cART initiation in high-income countries. © 2014 Wolters Kluwer Health | Lippincott Williams & Wilkins.

Abstract

We present two cases of tuberculous meningitis in Australian-born children. We review the current literature surrounding management of paediatric tuberculosis and disseminated disease, emphasising the importance of prompt diagnosis and intervention. We discuss the epidemiology of tuberculosis in the Australian paediatric population and highlight the sentinel role of childhood infection in public health surveillance. © 2012 The Authors. Abstract

The high resolution spectrograph (HRS) is designed to be used with the Large Space Telescope (LST) for the study of spectra of point and extended targets in the spectral range 110 to 410 nm. It has spectral resolutions of 1,000; 30,000; and 100,000 and has a field of view as large as 10 arc sec. The spectral range and resolution are selectable using interchangeable optical components and an echelle spectrograph is used to display a cross dispersed spectrum on the photocathode of either of 2 SEC orthicon image tubes. Provisions are included for wavelength calibration, target identification and acquisition and thermal control. The system considerations of the instrument are described.

Abstract

A technique is described for estimating the atmospheric radiance observed by a downward sensor (ERTS) using ground-based measurements. A formula is obtained for the sky radiance at the time of the ERTS overpass from the radiometric measurement of the sky radiance made at a particular solar zenith angle and air mass. A graph illustrates ground-based sky radiance measurements as a function of the scattering angle for a range of solar air masses. Typical values for sky radiance at a solar zenith angle of 48 degrees are given.

32. A technique for correcting ERTS data for solar and atmospheric effects

The author has identified the following significant results. Based on processing ERTS CCTs and ground truth measurements collected on Michigan test site for January through June 1973 the following results are reported: (1) atmospheric transmittance varies from: 70 to 85% in band 4, 77 to 90% in band 5, 80 to 94% in band 6, and 84 to 97% in band 7 for one air mass; (2) a simple technique was established to determine atmospheric scattering seen by ERTS-1 from ground-based measurements of sky radiance. For March this scattering was found to be equivalent to that produced by a target having a reflectance of 11% in band 4, 5% in band 5, 3% in band 6, and 1% in band 7; (3) computer ability to classify targets under various atmospheric conditions was determined. Classification accuracy on some targets (i.e. bare soil, tended grass, etc.) hold up even under the most severe atmospheres encountered, while performance on other targets (trees, urban, rangeland, etc.) degrades rapidly when atmospheric conditions change by the smallest amount.

33. An orbiting multispectral scanner for overland and oceanographic applications.

Abstract

Description of the major features of a multispectral scanner designed to perform overland and oceanographic surveys from space. The instrument uses an image plane conical scanner and contains independent spectrometers for land and ocean applications. The overland spectrometer has a spatial resolution of 200 ft and has six spectral bands in the atmospheric windows between 0.5 and 2.4 microns. The oceanographic spectrometer has a spatial resolution of 1200 ft and possesses 24 spectral bands equally spaced and in registration over the wavelength range from 0.4 to 0.8 micron. A thermal band of 600-ft resolution is used with a spectral range from 10.5 to 12.6 microns. The swath width of the scan is 100 nautical miles from an altitude of 500 nautical miles. The system has two modes of operation which are selectable by ground command. The six bands of overland data plus the thermal band data can be transmitted, or the 24 bands of oceanographic data plus data from two of the overland bands and the thermal band can be transmitted. The performance is described by the minimum detectable reflectance difference and the effects of sun angle and target reflectivity variations are discussed. The sensitivity is related to the variation of the ocean reflectivity in the presence of chlorophyll and to typical agricultural targets.

34. Machine processing of ERTS and ground truth data

Abstract

The author has identified the following significant results. Results achieved by ERTS-Atmospheric Experiment PR303, whose objective is to establish a radiometric calibration technique, are reported. This technique, which determines and removes solar and atmospheric parameters that degrade the radiometric fidelity of ERTS-1 data, transforms the ERTS-1 sensor radiance measurements to absolute target reflectance signatures. A radiant power measuring instrument and its use in determining atmospheric parameters needed for ground truth are discussed. The procedures used and results achieved in machine processing ERTS-1 computer -compatible tapes and atmospheric parameters to obtain target reflectance are reviewed.

A technique is described by which an ERTS investigator can obtain absolute target reflectances by correcting spacecraft radiance measurements for variable target irradiance, atmospheric attenuation, and atmospheric backscatter. A simple measuring instrument and the necessary atmospheric measurements are discussed, and examples demonstrate the nature and magnitude of the atmospheric corrections.
