Purpose: To evaluate postoperative ocular alignment at day 1 and at 6 months after unilateral LR recessionmedial rectus (MR) resection bilateral LR recession for Intermittent exotropia. Materials and Methods: Retrospectively medical records of 30 patients who underwent unilateral LR recession with MR resection and bilateral LR recession for intermittent exotropia were evaluated at postop day 1 & at 6 months. Each patient underwent complete orthoptic examination and had surgery plan for orthophoria. Surgical outcomes in reference of postoperative angle were compared & grouped as group A (esotropia 8-15 PD), group B (Orthotropia or esotropia/exotropia- 8 PD) & group C (exotropia>8-15 PD). Success alignment was defined as orthotropia or esotropia/ exotropia wihtin 6-8 PD. Results: Out of 30 patients, 19 underwent unilateral LR recession with MR resection & 11 underwent and bilateral LR recession. Pre-operative. Mean angle was 49.5(30-55 PD). Surgical outcome at post-operative day 1 in group A, B,C were 10%, 73.3%,16.7% respectively. Surgical outcome at 6 months in group A, B & C were 3.4%, 76.6% & 20% respectively. Surgical success at early ocular alignment and at 6 months 73.3% and 76.6% respectively. At 6 months success outcome in BLR was 81.8% & that in R/R was 73.6%. Conclusion: Early postoperative ocular alignment of intermittent exotropia at day 1 can be a determinant of orthophoria at 6 months. Orthophoric or slight overcorrection gives better ocular alignment but, prediction of long-term ocular alignment remains a grey zone. Purposes: To evaluate the relationship of changes in hypermetropia and ocular alignment in patients with accommodative esotropia. Methods: The medical records of consecutive patients diagnosed with refractive accommodative esotropia (esotropia eliminated or decreased to within 10 D with full hyperopic correction) were retrospectively reviewed. Cycloplegic refractions culled from medical records were converted into spherical equivalents. Presence of amblyopia, changes in refractive error and ocular alignment at admission and after the follow-up period were evaluated. Results: Seventy patients (mean age: 6.01 ± 5.41 years; female: 60.6%; mean follow-up: 5.8 ± 3 years) had corrected esotropia of 40 ± 20 prismatic diopters at admission. The average decrease per year in near and distance deviations with glasses was 1.71 ± 3.96 prismatic diopters/year and 1.09 ± 3.25 prismatic diopters/year, respectively. The total myopic shift of the right and left eyes was 1.08 ± 1.35 D and 1.20 ± 1.40 D, respectively. Myopic shift/year was 0.22 D/year and 0.26 D/year, respectively. The correlation between the rate of myopic shift and rate of change in corrected near deviation was weak. The correlation for the rate of myopic shift was not high for the right and left eyes (r=0.18; p=0.15). Conclusion: The amount of deviation and hypermetropia gradually decreased in accommodative esotropia during follow-up. On the other hand, it may be incorrect to assure patients that the amount of deviation will decrease in parallel with the refractive error. RESUMO Objetivo: Avaliar a relação entre alterações na hipermetropia e o alinhamento ocular em pacientes com esotropia acomodativa. Métodos: Foram analisados retrospectivamente prontuários médicos de pacientes consecutivos diagnosticados com esotropia acomodativa refrativa (com esotropia eliminada ou reduzida a menos de 10 D com correção completa da hipermetropia). As medidas de refração em cicloplegia obtidas dos prontuários foram convertidas em equivalentes esféricos. Avaliaram-se ainda a presença de ambliopia, alterações do erro refrativo e o alinhamento ocular à admissão e depois do período de acompanhamento. Resultados: Setenta pacientes (média de idade=6,01 ± 5,41 anos, 60,6% do sexo feminino, acompanhamento médio de 5,8 ± 3 anos) apresentaram esotropia de 40 ± 20 dioptrias prismáticas (DP) para perto à admissão. A diminuição média anual no desvio para perto e para longe com o uso de óculos foi de 1,71 ± 3,96 DP/ano e 1,09 ± 3,25 DP/ano, respectivamente. Os desvios miópicos totais dos olhos direito e esquerdo foram de 1,08 ± 1,35 D e 1,20 ± 1,40 D, respectivamente. Os desvios miópicos anuais foram de 0,22 D/ano e 0,26 D/ano para os olhos direito e esquerdo, respectivamente. A correlação entre a taxa de desvio miópico e a taxa de alteração do desvio para perto corrigido foi fraca. A correlação da taxa de desvio miópico não foi alta para os olhos direito e esquerdo (r=0,18, p=0,15). Conclusão: A quantidade de desvio e a hipermetropia diminuem gradualmente na esotropia acomodativa durante o acompanhamento. Por outro lado, pode não ser apropriado garantir aos pacientes que o desvio diminuirá em paralelo ao erro refrativo.

Parkinson's disease patients are classically described by having motor disorder symptoms. Freezing of gait is one of these motor symptoms that presents in some of these patients. Even though freezing of gait is classically considered as motor dysfunction, it is now widely accepted that deficits in other sensory systems, for example visual system, may lead or contribute to freezing of gait. The purpose of this study is to characterise some of the binocular vision functions in freezing of gait patients, non-freezing of gait Parkinson's disease patients and age-matched healthy controls.Binocular vision measurements included local and global stereopsis using different clinical stereo tests, along with fixation disparity and fixation disparity curves. The fixation disparity measures were primarily an assessment of the vergence motor system, and the clinical stereo tests assess the combined effects of motor and sensory aspects of binocular vision. Twenty-two freezing of gait patients, 25 non-freezing of gait patients, and 25 aged-matched healthy controls completed all of the measurements in this study.The freezing of gait group had worse stereopsis than the non-freezing of gait group, and the non-freezing of gait group had worse stereopsis than the healthy controls. The impairment of global stereopsis was more common than local stereopsis in Parkinson's disease patient groups. The reduction in stereopsis among Parkinson's disease patients was not associated with fixation disparity.Results from this study clearly indicate that the freezing of gait patients group has a greater loss in stereopsis than the other two groups, especially for the global tests. Whether these impairments are contributing to the freezing of gait or just associated with freezing of gait is uncertain.

Significance We sought to identify baseline and clinical factors that were predictive of the response to amblyopia treatment. We report that binocular amblyopia treatment may be especially effective for moderate amblyopia in orthotropic children. Purpose We previously reported results from the primary cohort (n = 28) enrolled in a randomized clinical trial (NCT02365090), which found that binocular amblyopia treatment was more effective than patching. Enrollment of an additional 20 children was pre-planned to provide the opportunity to examine factors that may be predictive of response to amblyopia treatment. Methods Forty-eight children (4 to 10 years old) were enrolled, with 24 randomized to contrast-rebalanced binocular game treatment (1 hour a day, 5 days a week) and 24 to patching treatment (2 hours a day, 7 days a week). The primary outcome was change in amblyopic eye best-corrected visual acuity at the 2-week visit. Baseline factors examined were age at enrollment, visual acuity, stereoacuity, and suppression. Clinical factors were etiology, age at diagnosis, prior treatment, and ocular alignment. Results At 2 weeks, visual acuity improvement was significantly greater with the binocular game than patching. Children with moderate amblyopia and orthotropia had more visual acuity improvement with binocular game play than did those with severe amblyopia. In addition, children who spent more time playing the binocular game had more improvement. We were not able to confidently identify any baseline or clinical factors that were associated with response to patching treatment. Conclusions Binocular amblyopia treatment was more effective among orthotropic children with moderate amblyopia than among children with microtropia or severe amblyopia.

A-pattern exotropia, superior oblique muscle overaction, and dissociated vertical deviation may coexist and are referred to as triad exotropia. The present study evaluated the postoperative stability of horizontal ocular alignment of triad exotropia and possible prognostic factors. Medical records of patients with triad exotropia who had one-step triple surgery of superior oblique muscle weakening, superior rectus muscle recession, and lateral rectus muscle recession were reviewed. The horizontal alignment and postoperative drift of triad exotropia were analyzed and compared with constant exotropia. The triad exotropia showed a mean of 7.7△ (± 8.5△) eso-drift, while the constant exotropia was (3.5△ ± 3.4△) exo-drift. Multiple linear regression analysis showed that the degree of superior oblique muscle overaction after surgery (P = 0.011) was the only factor associated with horizontal drift. Patients with superior oblique muscle underaction showed larger eso-drift when compared to patients without superior oblique muscle underaction (− 18.0△ ± 11.1△ vs. − 5.1△ ± 5.7△; P = 0.024). The final success rates of the triad exotropia and constant exotropia groups were 53.3% and 69.2%, respectively, and the overcorrection rates were 26.7% and 2.6% (P = 0.035). An overall trend of eso-drift in primary position occurred in triad exotropia after triple surgery up to a follow-up of 25 months. Patients presenting superior oblique muscle underaction after surgery seemed to have large angles of eso-drift, which might be taken into account in surgical planning and follow-up.

Background The Rebion blinq binocular birefringent ocular alignment screener was recently commercially released, but it did not yet have validation by American Association for Pediatric Ophthalmology and Strabismus (AAPOS) uniform guidelines. Methods Children and adults from a high-risk eye practice had screening by blinq with validation by AAPOS 2003 guidelines. Then, the blinq was compared to the Adaptica 2WIN with CR corneal reflex strabismus estimation by AAPOS 2003 guidelines plus additional efforts to identify patients with diminished binocularity. Results Blinq in 100 patients compared to 2003 AAPOS amblyopia risk factors (ARF) had sensitivity 67%, specificity 75% and PPV of 82%. Both blinq and 2WIN were completed by 87 patients median age 6.5 years. Sensitivity, specificity and positive predictive value (PPV) for blinq were 75%, 68% and 81% whereas 2WIN had 91%, 68% and 84%. The blinq referred two young patients with isolated, small-angle strabismic amblyopia that 2WIN refractive function passed. Conclusion Despite its non-refractive design to identify binocular foveation, blinq performed well with refractive and strabismic uniform risk factors and a PPV greater than 80%. In this study, we compare the effects of bifocal glasses with those of unifocal glasses in children with DS. Changes in angle of strabismus, accommodation and refractive error were analysed in this paper. METHODS: In a multicentre randomized controlled trial, 119 children with DS, aged 2-16, were randomly allocated for bifocal or unifocal glasses (with full correction of refractive error in cycloplegia). The 15 centres, all in the Netherlands, followed the participants for 1 year. Changes in refractive error, accommodative accuracy, strabismus, binocularity and stereopsis were compared across 4 subsequent visits. RESULTS: Refractive errors and accommodative errors showed no significant change throughout the course of our study in either intervention group. The manifest angle of strabismus, however, reduced significantly in the bifocal group. We investigated the effects of using a virtual reality smartphone-based head-mounted display (VR SHMD) device for 2 h on visual parameters. Fifty-eight healthy volunteers were recruited. The participants played games using VR SHMD or smartphones for 2 h on different days. Visual parameters including refraction, accommodation, convergence, stereopsis, and ocular alignment and measured choroidal thickness before and after the use of VR SHMD or smartphones were investigated. Subjective symptoms were assessed using questionnaires. We analyzed the differences in visual parameters before and after the use of VR SHMD or smartphones and correlations between baseline visual parameters and those after the use of the devices. Significant changes were observed in near-point convergence and accommodation, exophoric deviation, stereopsis, and accommodative lag after the use of VR SHMD but not after that of smartphones. The subjective discomfort associated with dry eye and neurologic symptoms were more severe in the VR group than in the smartphone group. There were no significant changes in refraction and choroidal thickness after the use of either of the two devices. The poorer the participants’ accommodation and convergence ability the greater the resistance to changes in these visual parameters, and participants with a large exophoria were more prone to worsening of exophoria than those with a small exophoria.

PurposeTo study the longitudinal rehabilitation of binocular visual function in adolescent intermittent exotropia (IXT) after successful surgery and compare the results with those of a normal population. The role of binocular function in ocular alignment stability was also evaluated postoperatively.MethodsIn this prospective study, 30 adolescents with IXT successfully corrected after 1 month were followed for 12 months, and 30 children with normal vision were enrolled as controls. Stereopsis, the fusional vergence amplitude, sensory fusion, and accommodative flexibility were measured to assess binocular function at baseline and 6 and 12 months postoperatively. The controls were tested once when they were enrolled in the study.ResultsThe deviation was −32.00 ± 8.60 prism diopters (PD) at distance fixation and −36.0 ± 9.10 PD at near fixation preoperatively with an average correction of 28.53 ± 3.79 PD and 30.67 ± 1.34 PD at 1 month postoperatively. Distance stereoacuity and near stereoacuity improved from 1 to 12 months postoperatively (p = 0.025 and p = 0.041, respectively). Compared with the controls, the fusional convergence reserve at distance (p = 0.025) and near (p = 0.033) fixations and fusion reserve ratio at distance (p = 0.000) and near (p = 0.000) fixations remained subnormal, whereas sensory fusion (p = 0.237), distance stereopsis (p = 0.120), and the fusional divergence amplitude at a distance (p = 0.168) were normal. However, no significant correlations were found between binocular functions at 1 month postoperatively and the postoperative drift.ConclusionBinocular function significantly improved from before to after successful corrective surgery and continued to improve from 1 to 12 months postoperatively in adolescents with IXT. No significant correlations were found between binocular functions at 1 month postoperatively and ocular alignment stability.

Previous series suggest adjustable sutures (AS) in adult strabismus surgery yield improved ocular alignment and better success rates compared to nonadjustable sutures (NAS). We questioned whether these differences are clinically significant and whether they justify the added time and discomfort required for AS.We reviewed all available records of adults undergoing horizontal strabismus surgery by the last two authors between 2000 and 2014. Independently, the two surgeons developed a preference for NAS midway through the study period, permitting comparisons between two treatment groups. Results were assessed at one to two months postoperatively. The primary outcome was alignment in primary position at one to two months postoperatively. The secondary outcome was success rate, defined as10PD residual or consecutive deviation.We included 184 patients, 68 with AS and 116 with NAS. No significant difference in primary position alignment at 1-2 months was noted between AS and NAS for esotropia (Although we acknowledge limitations in this retrospective study, our results suggest that AS overall was not associated with improved alignment or success rates, compared to NAS, at 1- to 2- months postoperatively.Although adjustable sutures represent a valuable surgical option at the discretion of individual surgeons and their patients, we no longer routinely use AS in all adult cases. A prospective study to evaluate long-term outcomes would be helpful.

A smartphone app has been developed to perform the automated photographic Hirschberg test for objective measurement of ocular misalignment. By computing the difference in corneal reflection generated by the phone camera flash relative to the iris center based on high resolution images, the app can measure misalignment with a much higher precision than the naked eye performing the Hirschberg test. It has been validated in a previous clinical evaluation study by comparing to the clinical gold standard-prism and alternate cover test. The goal of this article is to describe the testing techniques regarding how to use the app to measure ocular alignment for different fixation distances, without or with cover to break fusion, as well as angle kappa, so that users can use the app to perform equivalent tests typically done in clinic using prisms.

Purpose To evaluate the clinical characteristics of patients with intermittent exotropia who exhibited a dramatic decrease in ocular deviation 1 day before surgery. Methods This study retrospectively enrolled patients with intermittent exotropia who underwent surgery between December 2013 and December 2019. Those who exhibited a decrease in ocular alignment ≥10 prism diopters (PD) at the last examination (1 day before surgery) compared with the largest previous angle of deviation were included. A monocular occlusion test was performed to re-confirm the largest angle of ocular deviation. Results Among 547 patients with intermittent exotropia, 10 (six females; mean age, 8.6 years) exhibited a dramatic decrease in ocular deviation before impending surgery. The mean largest angle of deviation was 31.0 PD (range, 20 to 50 PD) at distance and 34.5 PD (range, 20 to 55 PD) at near. The mean control scores using the LACTOSE (Look and Cover then Ten Seconds of Observation Scale for Exotropia) scoring system were 2.5 at distance, 1.8 at near, and 4.3 when combined. Mean ocular deviation prior to the impending surgery decreased to 7.4 PD (range, 0 to 10 PD) at distance and 6.2 PD (range, -10 to 10 PD) at near. The largest angle of ocular deviation was re-confirmed using the monocular occlusion test in all patients. All patients underwent surgery as planned, and none exhibited postoperative overcorrection. Conclusions A relatively small number of patients with intermittent exotropia exhibited a dramatic decrease in ocular alignment 1 day before surgery but demonstrated a relatively better level of control. The monocular occlusion test was helpful in re-confirming the largest angle of ocular deviation.

Purpose: To investigate the efficacy of a modified surgical procedure for esotropia fixus with high myopia. Methods: Thirteen patients (15 eyes) with esotropia fixus and high myopia who underwent the Jensen procedure for superior and lateral rectus muscles at Southwest Hospital between February 2014 and December 2015 were retrospectively analyzed. Intraoperatively, the superior rectus and lateral rectus muscles were separated up to 12 to 14 mm posterior to their respective insertion. A medial rectus large recession or rectus tenotomy was performed based on the degree of fibrosis of the medial rectus muscle. Postoperative examinations were performed at 1 day, 2 weeks, 3 months, and 6 months. Results: On the first postoperative day, 12 eyes (10 patients) were in the primary position (80.0%), 2 eyes (2 patients) exhibited 5° to 10° esotropia (13.3%), and 1 eye (1 patient) exhibited 15° esotropia (6.7%). At the 2-week follow up, 1 eye (1 patient) was lost to follow-up, 9 eyes (7 patients) were in the primary position (64.3%), 3 eyes (3 patients) exhibited 10° esotropia (21.4%), and 2 eyes (2 patients) exhibited 15° to 20° esotropia (14.3%). At the 3-month follow-up, the patient whose ocular alignment was 20° esotropia at 2 weeks was found to have developed 30° esotropia; no change was observed in the other patients. A remarkable improvement in ocular motility was observed in all patients. Conclusions: The Jensen procedure for the union of the superior rectus and lateral rectus muscles, using two pairs of sutures applied 12 to 14 mm posterior to their respective insertions, yielded favorable outcomes. BACKGROUND We have developed a non-invasive, behavioral measure of ocular alignment using a computer tablet, colored lenses, and touch screen software. OBJECTIVE The goal of this study was to determine if these tests differentiate healthy controls from patients with vestibular disorders. METHODS In the vertical alignment nulling test (VAN), subjects were asked to adjust a horizontal line that was offset vertically from a fixed horizontal line. In the torsional alignment nulling test (TAN) subjects were asked to adjust a line that was rotationally offset (i.e. clockwise) from a fixed horizontal line. We measured VAN and TAN in 14 healthy controls and 8 patients with known vestibular disorders. RESULTS Patients had significantly worse scores than controls on TAN, (mean 2.2 vs 0.75, p = 0.01), and no differences for scores compared to controls on VAN, (mean 0.4 vs 0.8, p = 0.07). CONCLUSIONS These results suggest that TAN, and possibly VAN, have potential for identifying misalignments in ocular position. After further technical development these tests might be useful in the future for screening patients in facilities that are not equipped to perform cervical and ocular vestibular evoked myogenic potentials.

Minimally invasive surgery is rapidly becoming the norm in medicine, as it often leads to better outcomes and earlier rehabilitation. This article reviews the principles and different techniques employed to perform minimally invasive strabismus surgery (MISS). In these techniques, strabismus surgery is performed through keyhole openings, thus reducing the risk of postoperative corneal complications, minimizing postoperative discomfort, and better preserving muscle function. MISS can be used to perform all types of strabismus surgery, namely rectus muscle recessions, resections, plications, reoperations, retroequatorial myopexy, transpositions, oblique muscle recessions, or plications even in the presence of limited motility. Of note, ocular alignment outcomes with MISS versus more traditional techniques have not been compared in randomized trials. Consequently, more controlled evidence is still needed to better delineate the future role and value of MISS in clinical management.

Strabismus is an ophthalmological disease characterized by an imbalance in the ocular muscles, characterized by the deviation of the binocular alignment, in which the eyes can turn inwards, outwards, upwards or downwards. Its incidence can occur from the first months of life to adulthood. Thus, late detection may cause double vision or loss of vision, hence the importance of early detection of strabismus for its prognosis and effective treatment. In this work, we propose a fully automatic method to detect strabismus by evaluating the distance between the limb centroid and the point in between the eye corners. The proposed method achieved an accuracy of 96.6% with a sensitivity of 95.8% and a specificity of 100%.

Strabismus is a pathology that affects the parallelism between two eyes. Estimates show that 1.3% to 3.5% of children aged six months to six years have strabismus around the world. Strabismus can lead to irreversible loss of vision, so early detection and appropriate treatment increase the likelihood of alignment being restored to normal. That said, in this study we proposed an automatic evaluation system to detect strabismus in face images, analyzing the horizontal positioning of the limb center in relation to the center of the corners of the eyes. The proposed method consists of five steps: (1) acquisition, (2) detection of the eye region, (3) segmentation and reconstruction of the sclera, (4) detection of the limb region and corners of the eyes and (5) identification of the presence of strabismus, with this approach we obtain 90.1% sensitivity, 100% specificity and 91.1% accuracy.

Purpose To observe the change in horizontal smooth pursuit in patients with intermittent exotropia before and after strabismus surgery. Methods The smooth pursuit of patients with intermittent exotropia was recorded using a video eye-tracker before and 1 week, and 1, 3, and 6 months after strabismus surgery. Subjects were asked to track a step-ramp target moving at ±6.1°/sec horizontally as accurately as possible under binocular viewing. The differences in gain (eye velocity divided by target velocity) and amplitude of smooth pursuit between right and left eyes before and after surgery were compared. Results A total of 9 patients (mean age, 22.2 ± 13.9 years) were included. Mean preoperative ocular alignment at near was 59.1Δ ± 34.7Δ. The difference in gain between the left and right eyes before surgery was 0.23 ± 0.1; in amplitude, 3.0° ± 3.7°. These values were improved at 1 week after surgery (gain, 0.08 ± 0.06; amplitude, 0.9° ± 0.65°) and continued to improve for 6 months after surgery; however, ocular alignment at near reverted from 5.9Δ ± 10.5Δ to 18.9Δ ± 17.5Δ by 6 months after surgery. Conclusions In this small patient cohort, surgical correction of ocular alignment improved binocular coordination of smooth pursuit in intermittent exotropia. In the long term, the improvement of the binocular coordination of smooth pursuit tended to remain, although the eye position partially reverted to the presurgical state.

The evidence-basis based on existing myopia control trials along with the supporting academic literature were reviewed; this informed recommendations on the outcomes suggested from clinical trials aimed at slowing myopia progression to show the effectiveness of treatments and the impact on patients. These outcomes were classified as primary (refractive error and/or axial length), secondary (patient reported outcomes and treatment compliance), and exploratory (peripheral refraction, accommodative changes, ocular alignment, pupil size, outdoor activity/lighting levels, anterior and posterior segment imaging, and tissue biomechanics). The currently available instrumentation, which the literature has shown to best achieve the primary and secondary outcomes, was reviewed and critiqued. Issues relating to study design and patient selection were also identified. These findings and consensus from the International Myopia Institute members led to final recommendations to inform future instrumentation development and to guide clinical trial protocols.

BACKGROUND The gradual decrease in binocularity and stereopsis in patients with concomitant strabismus decreases the depth perception of the patients and adversely affects the reading speed and the motor dexterity skills of the individuals. It had been observed earlier that early management is necessary to help patients regain their binocularity, but recent studies indicate that binocularity and stereopsis may improve in these patients even if they are treated later in life, provided they are managed well. Hence, we undertook this study to determine the extent of improvement in binocularity and stereopsis in patients with long-standing large angle constant concomitant horizontal strabismus after surgical management after the age of 16 years and its association with the postoperative alignment. MATERIALS AND METHODS A prospective clinical trial was conducted in 30 patients with constant concomitant horizontal strabismus of more than 25 prism dioptres who presented after the age of 16 years. Binocularity and stereopsis were measured in the patients with the help of Worth 4-dot test, synoptophore, Lang II test plate, Randot test plates and TNO test plates. The measurements were done preoperatively and 6 weeks after surgical treatment. The changes in the measurements were associated with the postoperative alignment. RESULTS Preoperatively, the patients demonstrated either alternating or unilateral suppression with no measurable stereopsis. Postoperatively, satisfactory surgical alignment defined as tropia less than or equal to 8 Prism Dioptres (PD) was achieved in 23 (76.6%) of the patients. 19 (63.3%) patients achieved binocularity on Worth 4- dot test and synaptophore, but alternating or unilateral suppression was still present in 11 (36.6%) of the patients. 17 (56.6%) of the 30 patients achieved stereopsis on Lang II, Randot and TNO test plates. Development of stereopsis was associated with postoperative alignment with 16 (69.5%) of the 23 satisfactorily aligned patients and only 1 (14.3%) of the 7 patients with a residual deviation of ≥10PD achieving stereopsis. CONCLUSION Significant improvement in binocularity and stereopsis can be achieved in patients with long-standing large angle constant concomitant horizontal strabismus after surgical management even after the age of 16 years and the development of binocularity and stereopsis is associated with the final postoperative alignment.

The European Neuromuscular Centre (ENMC) derived the term Congenital Cranial Dysinnervation Disorders in 2002 at an international workshop for a group of congenital neuromuscular diseases. CCDDs are congenital, non-progressive ophthalmoplegia with restriction of globe movement in one or more fields of gaze. This group of sporadic and familial strabismus syndromes was initially referred to as the 'congenital fibrosis syndromes' because it was assumed that the primary pathologic process starts in the muscles of eye motility. Over the last few decades, evidence has accumulated to support that the primary pathologic process of these disorders is neuropathic rather than myopathic. This is believed that for normal development of extra ocular muscles and for preservation of muscle fiber anatomy, normal intra-uterine development of the innervation to these muscles is essential. Congenital dysinnervation to these EOMs can lead to abnormal muscle structure depending upon the stage and the extent of such innervational defects. Over last few years new genes responsible for CCDD have been identified, permitting a better understanding of associated phenotypes, which can further lead to better classification of these disorders. Introduction of high-resolution MRI has led to detailed study of cranial nerves courses and muscles supplied by them. Thus, due to better understanding of pathophysiology and genetics of CCDDs, various treatment modalities can be developed to ensure good ocular alignment and better quality of life for patients suffering from the same.

Managing large-angle exotropia (>70 PD) can be challenging with the various techniques available. In this case series, we present “True Muscle Transplantation"––a simple technique––to manage large-angle sensory exotropia as a possible alternative. Seven patients (four women and three men) of mean age 39.71 ± 17.38 years with sensory large-angle exotropia (>70 PD) underwent proposed surgery after detailed preoperative workup. The stump resected from MR was transplanted to LR and recessed. The mean preoperative deviation of 77.14 ± 4.52 PD base-in reduced to mean postoperative angle of 4.71 ± 4.42 PD at 6 months with good ocular alignment, no lateral incommittance, and no limitation of ductions in all patients. Stable results were maintained beyond 6 months. True Muscle Transplantation, therefore, has the potential of being a possible alternative.

Ocular alignment defects such as strabismus affect around 5% of people and are associated with binocular vision impairments. Current nonsurgical treatments are controversial and have high levels of recidivism. In this study, we developed a rehabilitation method for ocular alignment training and examined the rate of learning, transfer to untrained alignments, and retention over time. Ocular alignment was controlled with a real-time dichoptic feedback paradigm where a static fixation target and white gaze-contingent ring were presented to the dominant eye and a black gaze-contingent ring with no fixation target was presented to the nondominant eye. Observers were required to move their eyes to center the rings on the target, with real-time feedback provided by the size of the rings. Offsetting the ring of the nondominant temporal or nasal visual field required convergent or divergent ocular deviation, respectively, to center the ring on the fixation target. Learning was quantified as the time taken to achieve target deviation of 2° (easy, E) or 4° (hard, H) for convergence (CE, CH) or divergence (DE, DH) over 40 trials. Thirty-two normally sighted observers completed two training sequences separated by one week. Subjects were randomly assigned to a training sequence: CE-CH-DE, CH-CE-DE, DE-DH-CE, or DH-DE-CE. The results showed that training was retained over the course of approximately one week across all conditions. Training on an easy deviation angle transferred to untrained hard angles within convergence or divergence but not between these directions. We conclude that oculomotor alignment can be rapidly trained, retained, and transferred with a feedback-based dichoptic paradigm. Feedback-based oculomotor training may therefore provide a noninvasive method for the rehabilitation of ocular alignment defects.

The target is selected according to the purpose of the test. If it is necessary to test in accommodative environment, a small accommodating target should be used. And a large target or a light source may be used if the accommodation is to be eliminated. In the case of infants, a target that can induce the interest of the infant is used. Cover test can only be performed on patients who have enough visual acuity and concentration ability to look at the target. In addition, the results are inaccurate even when there is an abnormal retinal correspondence and extrafoveal fixation. The eye level of the examinee should be the same as the patient and be careful not to occlude the target by the examiner. Since the head position of the patient may affect the rest position, examination is required at the right head position.

It is often perplexing to see a patient with deviated eyes and random-appearing eye movements. ‘The strabismus puzzle’ can be a daunting task for anyone in the absence of lucid understanding of the motor and sensory mechanisms that govern ocular alignment and movement.

Aim To describe the etiology, clinical characteristics, surgical options and surgical outcomes of isolated inferior oblique palsy (IOP). Methods A retrospective review was performed on patients with isolated IOP who were seen between January 2010 and June 2017. The following clinical data were obtained from the patients' charts: visual acuity, ocular alignment, ocular motility, cyclotorsion, stereoacuity, Parks three-step test, surgical methods, surgical outcomes and complications. Surgical success was defined as horizontal deviation ≤10 prism diopters (PD) and a vertical deviation ≤5 PD in primary gaze at both near and distant vision as assessed at last follow-up. Results The records from a total of 18 patients (8 males and 10 females) with an average age of 27.56y were included in this study. The right eye was affected in 11 patients, the left in 6 patients and both eyes in 1 patient. Twelve cases (66.7%) were congenital and 6 (33.3%) were acquired IOP. The 6 acquired cases involved 2 resulting from orbital trauma/surgery, 2 from midbrain microvascular ischemia, 1 from myasthenia gravis and 1 of unknown etiology. Strabismus surgery was performed in 13 cases. Surgical techniques included weakening of superior oblique and vertical rectus recession and resection. After a mean follow-up of 15.11mo, the corrected vertical deviation in primary position was 19.92±8.52 PD (P=0.000) and the corrected horizontal deviation was 14.31±12.68 PD (P=0.002). The surgical success rate was 61.5% and no surgical complications were present. Conclusion Isolated IOP represents a rare condition, with most cases (66.7%) involving a congenital basis. The acquired cases included vascular, orbital trauma/surgery and myasthenia gravis. Weakening of the ipsilateral superior oblique muscle and/or contralateral superior rectus recession often resulted in favorable surgical outcomes with a surgical success rate of 61.5%.

Purpose StrabisPIX is a smartphone application that allows clinicians to prescribe a series of self-obtained images of head position and eye alignment in nine positions of gaze that are uploaded onto a secure platform for clinician review. This study evaluates the clinical utility of this application. Methods In this prospective, nonmasked, cross-sectional study, 30 strabismus patients aged ≥2 years were evaluated. Participants received standardized instructions, used StrabisPIX to obtain images as prompted, and completed a satisfaction survey. During the same visit, an orthoptist obtained standard clinical images with a professional camera. All 60 image sets were evaluated by three observers. Results StrabisPIX image quality was similar to that of clinic photographs. Clinic photographs had significantly higher acceptability for horizontal versions (81% vs. 67%), vertical versions (76% vs. 60%), and head posture (93% vs. 81%). Abnormalities were detected at a similar rate for versions, head posture, eyelid position, and pupil size. StrabisPIX had significantly higher detection of alignment abnormalities (89% vs. 77% for clinical photos). Interrater/intrarater agreements were moderate to high (κ = 0.44-1.00) for all parameters except pupil abnormality, which had poor to fair agreement. Most patients reported that StrabisPIX was easy to learn and use. Conclusions Overall, StrabisPIX images had similar quality and were as useful as images obtained in the clinic in detecting abnormalities. Translational relevance The StrabisPIX application will enhance the use of telemedicine by allowing physicians to prescribe self-obtained images documenting strabismus.

To evaluate the results of ipsilateral inferior rectus (IR) recession, when performed alone or in combination with contralateral superior rectus (SR) recession in patients with monocular elevation deficiency (MED) and IR tightness.The medical records of patients with MED and tight IR muscle were retrospectively reviewed. Patients underwent either ipsilateral IR recession alone (IR group) or combined with contralateral SR recession (IR+SR group). Pre- and postoperative ocular motility and alignment and fundus torsion were analyzed. Abnormal head posture was recorded. All patients were followed for at least 6 months.A total of 23 patients were identified. Mean patient age at time of surgery was 9.8 ± 8.7 years (range, 3-45 years). Ten patients underwnet ipsilateral IR recession 5-8 mm; 13 patients, 4 mm ipsilateral IR recession combined with 5-12 mm contralateral SR recession. Both groups experienced marked improvement in ocular alignment in the primary position (P = 0.34). However, there was a statistically better ocular alignment in both upgaze (P = 0.03) and downgaze (P0.01) in the IR+SR group, with a lower degree of fundus intorsion (P0.01). An ipsilateral head tilt developed in 70% of patients in the IR group and in 54% of patients in the IR+SR group.In this patient cohort, combined contralateral SR recession with ipsilateral IR recession reduced postoperative IR underaction and achieved better ocular alignment in upgaze and downgaze.

Purpose- The aim of the study is to compare visual functions of children with uncorrected amblyopia treated with different binocular I Pad games and/or patching. Methods- Children with untreated amblyopia were enrolled in this pilot study. Children were divided into three groups: children treated with patching only, children attributed to play binocular snake I Pad game with patching, children attributed to play binocular blocks I Pad game with patching. Visual acuity VA, refraction, stereo acuity test, and ocular alignment tests were performed for all participants in the first visit and 4 weeks after starting treatment. Visual functions were compared between both visits. Results- A total of 18 children with untreated amblyopia participated in this study. Children were divided into three groups: Group 1= 8 children treated with patching only, Group 2= 5 children attributed to play binocular snake I Pad game with patching, and Group 3= 5 children attributed to play binocular blocks I Pad game with patching. There was no significant difference (P > 0.05) in gender and age between the three groups. Children showed good compliance to all patching treatment and I pad games groups. After 4 weeks, Visual acuity was improved in all amblyopic children in the form of one line improvement in patching group, two lines with binocular snake I Pad game and three lines with binocular blocks I Pad game. Moreover, stereoacuity was improved in all groups with greatest advance in blocks games group followed by snake group then patching group. Conclusion- This study demonstrated great effectiveness of both snake, and blocks binocular I Pad games on visual functions improvement for treatment of amblyopia. These games can contribute with patching in achieving better VA and stereoacuity enhancement in a shorter duration than patching alone.
