Retrospective studies and single center experiences suggest a role of capecitabine combined with temozolomide (CAPTEM) in neuroendocrine tumors (NENs).We performed a systematic review to assess the efficacy and safety of CAPTEM in patients affected with NENs, with the aim to better clarify the role of this regimen in the therapeutic algorithm of NENs.A total of 42 articles and 1818 patients were included in our review. The overall disease control rate was 77% (range 43.5%-100%). The median progression free survival ranged from 4 to 38.5 months, while the median overall survival ranged from 8 to 103 months. Safety analysis showed an occurrence of G3-G4 toxicities in 16.4% of the entire population. The most common toxicities were hematological (27.2%), gastrointestinal (8.3%,) and cutaneous (3.2%).This systematic review demonstrated that CAPTEM was an effective and relatively safe treatment for patients with advanced well-moderate differentiated NENs of gastroenteropancreatic, lung and unknown origin.

Background: Sedentary behaviors have increased in recent years and their consequences have led the World Health Organization to make recommendations for promoting a more active lifestyle. The school environment has been defined as a key place for achieving this objective for children and adolescents. This systematic review and meta-analysis aims to analyze the effect of active-break interventions for interrupting prolonged sitting times during school-time on physical activity (PA) and sedentary behavior (SB), at school, in childhood and youth. Methods: A systematic review and meta-analysis were carried out, including clinical trials aimed at assessing the effects of interrupting prolonged sitting at school with active breaks on in-school PA and/or SB. Multimodal and static interventions were excluded. Six databases were analyzed: Medline, WOS, Cochrane Library, SPORT Discus, CINAHL and EMBASE. PA, SB; moderate-to-vigorous physical activity (MVPA) were the variables considered. Results: Nine studies were included, with a total of 2145 children between 6 and 12 years old. The heterogeneity in the duration (five–sixty min), the frequency (one–three times per-day up to three times per week), and duration (five days to three years) of the interventions was detected. The meta-analyses for in-school PA, MVPA, and SB were performed, showing a significant improvement in both PA and MVPA. Conclusions: Interrupting prolonged sitting with active-based school interventions could improve PA and MVPA levels during school time. (PROSPERO: CRD42022358933).

El present article intenta evidenciar punts de convergència i divergència entre la representació dels nens en l'art africà i en la producció cinematogràfica de ficció del realitzador de Guinea Bissau Flora Gomes. En primer lloc, de manera molt sintètica, s'exposen aquelles tipologies plàstiques on apareix el nen (figuretes com a part de rituals iniciàtics, representació de les maternitats i parelles de bessons). A continuació, s'identifica on apareixen aquestes temàtiques, i com són abordades als films., This article intends to show points of convergence and divergence between children representation in African art and fiction feature film by Flora Gomes, filmmaker from Guinea Bissau. First, are displayed in general lines those African art typologies in which children are represented (some figurine used in initiation rituals, motherhood representation and couple of twins). Later are identified these topics in those films and how they are treated by the filmmaker., El presente artículo intenta evidenciar puntos de convergencia y divergencia entre la representación de los niños en el arte africano y en la producción cinematográfica de ficción del realizador de Guinea Bissau Flora Gomes. Primeramente, de manera muy resumida se exponen aquellas tipologías plásticas donde aparece el niño (figurillas como parte de rituales iniciáticos, representación de las maternidades y parejas de gemelos). A continuación se identifican dónde aparecen esas temáticas, y cómo son abordadas en los filmes.

COVID-19 affects children to a lesser extent than adults but they can still get infected and transmit SARS-CoV-2 to their contacts. Field deployable non-invasive sensitive diagnostic techniques are needed to evaluate the infectivity dynamics of SARS-CoV-2 in pediatric populations and guide public health interventions, particularly if this population is not fully vaccinated. We evaluated the utility of high-throughput Luminex assays to quantify saliva IgM, IgA and IgG antibodies against five SARS-CoV-2 spike (S) and nucleocapsid (N) antigens in a contacts and infectivity longitudinal study in 122 individuals (52 children and 70 adults). We compared saliva versus serum/plasma samples in infected children and adults diagnosed by weekly RT-PCR over 35 days (n=62), and those who consistently tested negative over the same follow up period (n=60), in the Summer of 2020 in Barcelona, Spain. Saliva antibody levels in SARS-CoV-2 RT-PCR positive individuals were significantly higher than in negative individuals and correlated with those measured in sera/plasmas. Asymptomatic infected individuals had higher levels of anti-S IgG than symptomatic individuals, suggesting a protective anti-disease role for antibodies. Higher anti-S IgG and IgM levels in serum/plasma and saliva, respectively, in infected children compared to infected adults could also be related to stronger clinical immunity in them. Among infected children, males had higher levels of saliva IgG to N and RBD than females. Despite overall correlation, individual clustering analysis suggested that responses that may not be detected in blood could be patent in saliva, and vice versa. In conclusion, measurement of SARS-CoV-2-specific saliva antibodies should be considered as a complementary non-invasive assay to serum/plasma to determine COVID-19 prevalence and transmission in pediatric populations before and after vaccination campaigns. L’etapa de Preescolar és un dels moments més importants en la vida de l’ésser humà; és allí on els nens presenten avanços significatius en totes les dimensions, però especialment desenvolupen la part socioemocional en el moment en què interactuen amb l’altre: l’empatia i l’amor propi. També s’emmarquen les habilitats d’autocontrol i autoregulació, les quals tenen gran significat en aquesta etapa, ja que s’enfoquen en què els nens siguin capaços de regular el que pensen i senten, distingint, a partir d’això, la manera com responen davant qualsevol situació que se’ls presenti en la vida. Per mitjà d’aquest article es proposa desplegar una mirada general sobre les habilitats d’autocontrol i autoregulació en l’etapa Preescolar; es va realitzar una revisió narrativa, on es va tenir en compte literatura científica que permetés conèixer els estudis on s’hagués avaluat l’autoregulació i l’autocontrol en nens. Segons les troballes de la revisió literària, es presenten els resultats tenint en compte les següents categories: conceptualització de les habilitats d’autocontrol i autoregulació, autoregulació en la infància i estils parentals, l’autoregulació i el joc. Per mitjà d’aquest recorregut teòric i partint de la indagació motivant respecte al reconeixement de les habilitats d’autocontrol i autoregulació en l’edat preescolar, s’aconsegueix desprendre la importància de reconèixer i concebre la diferenciació entre els conceptes d’autoregulació i l’autocontrol, en tant que des de la seva singularitat aconsegueixen establir importants eines des dels primers anys de vida per a aconseguir cognitivament un domini en l’obtenció de metes i l’adaptació al mitjà; aspectes que contribueixen a l’aprenentatge., The Pre-school stage is one of the most important moments in human life; it is there that children present significant advances in all dimensions, but especially develop the socio-emotional part of the time they interact with each other: empathy and love of their own. Self-monitoring and self-regulation skills, which are of great significance at this stage, are also framed, as they are aimed at children being able to regulate what they think and feel, thereby distinguishing, from this, the way they respond to any situation they are presented with in life. Through this article, it is proposed to take a general look at the skills of self-control and self-regulation at the Pre-school stage; a narrative review was carried out, where scientific literature was considered that would allow studies to be known where self-regulation and self-control in children had been assessed. According to the findings of the literary review, the results are presented considering the following categories: conceptualization of self-control and self-regulation skills, child self-regulation and parental styles, self-regulation and gambling. Through this theoretical approach and on the basis of the motivating analysis with regard to the recognition of self-control and self-regulation skills in pre-school age, it is possible to disregard the importance of recognising and conceiving the differentiation between self-regulation concepts and self-control, while from their uniqueness they manage to establish important tools from the earliest years of life in order to cognitively achieve a mastery in achieving goals and adapting to the environment; aspects that contribute to learning., La etapa del preescolar es uno de los momentos más importantes en la vida del ser humano; es allí donde los niños presentan avances significativos en todas las dimensiones, pero en especial desarrollan la parte socio emocional en el momento en que interactúan con el otro, la empatía y el amor propio. También se enmarcan las habilidades de autocontrol y autorregulación, las cuales tienen gran significado en esta etapa ya que se enfocan en que los niños sean capaces de regular lo que piensan y sienten, distinguiendo a partir de ello, el modo como responden ante cualquier situación que se les presente en la vida. Por medio de este artículo se propone desarrollar una mirada general sobre las habilidades de autocontrol y autorregulación en la etapa del preescolar, se realizó una revisión narrativa, donde se tuvo a consideración literatura científica que permitiera conocer sobre estudios donde se hubiera evaluado la autorregulación y el autocontrol en niños. Según los hallazgos de la revisión literaria se presentan los resultados teniendo en cuenta las siguientes categorías: conceptualización de las habilidades de autocontrol y autorregulación, autorregulación en la infancia y estilos parentales y autorregulación y el juego. Por medio de este recorrido teórico y partiendo de la indagación motivante respecto al reconocimiento de las habilidades de autocontrol y autorregulación en la edad preescolar, se logra desprender la importancia de reconocer y concebir la diferenciación entre los conceptos de autorregulación y el autocontrol, en tanto, que desde su singularidad, logran establecer importantes herramientas desde los primeros años de vida para alcanzar cognitivamente un dominio en la obtención de metas y la adaptación al medio, aspectos que contribuyen en el aprendizaje.&nbsp

El projecte consta d'un nou equipament educatiu a Castellar del Vallès, situat en un punt estratègic i adaptat a la complexa topografia tot generant una porta d'entrada (edifici pont) a la natura i oferint un nou passatge públic. Es tracta d'un espai complementari a les escoles existents de la ciutat, el qual oferirà un programa educatiu alternatiu que promou un aprenentatge vinculat amb la natura i el món artístic per a tots els alumnes, tant de la ciutat com d'altres municipis, potenciant doncs, la relació entre els nens/es de diferents edats i escoles. A més, el programa inclou altres col·lectius (grups d'adults de Castellar) que podran dur a terme un seguit d'activitats que actualment no tenen lloc i que formaran part del nou teixit relacional de l'educació dels infants. El proyecto consta de un nuevo equipamiento educativo en Castellar del Vallès, situado en un punto estratégico y adaptado a la compleja topografía generando una puerta de entrada (edificio puente) en la naturaleza y ofreciendo un nuevo pasaje público. Se trata de un espacio complementario a las escuelas existentes de la ciudad, que ofrecerá un programa educativo alternativo que promueve un aprendizaje vinculado con la naturaleza y el mundo artístico para todos los alumnos, tanto de la ciudad como de otros municipios, potenciando la relación entre los niños/as de diferentes edades y escuelas. Además, el programa incluye a otros colectivos (grupos de adultos de Castellar) que podrán llevar a cabo una serie de actividades que actualmente no tienen lugar y que formarán parte del nuevo tejido relacional de la educación de los niños. The project consists of new educational equipment in Castellar del Vallès, located at a strategic point and adapted to the complex topography, generating a bridge building inside the nature and offering a new public passageway. It is a complementary space to the existing schools in the city, which will offer an alternative educational program that promotes learning linked to nature and the artistic world for all students, both from the city and from other, strengthening the relationship between children of different ages and schools. In addition, the program includes other groups (adult groups from Castellar) that will be able to carry out a series of activities that currently do not take place and that will form part of the new relational network of children's education.

El consumo de bebidas energéticas o energizantes ha alcanzado una gran extensión a nivel mundial, especialmente entre adolescentes y niños, lo que constituye una auténtica problemática en materia de salud pública, por lo que organismos internacionales de renombre como la Organización Mundial de la Salud o la Agencia Española de Seguridad Alimentaria y Nutrición han desarrollado diversas investigaciones al respecto. Estas bebidas energéticas se caracterizan por poseer una composición similar de ingredientes (i.e., cafeína, guaraná, taurina, aminoácidos, glucuronolactona y otros azúcares, ginseng, vitaminas del grupo B y extractos herbales entre otros), si bien pueden diferir enormemente en cuanto a dosis y cantidades de cada componente ya que no existe una regulación uniforme sobre la composición, venta, promoción y distribución de éstas. El consumo de estas bebidas se ha asociado a beneficios como, incremento de la energía disponible, mejora del rendimiento físico o un retraso en la aparición de la fatiga y el sueño, aunque es importante reseñar que actualmente no hay un criterio uniforme en cuanto a los efectos asociados a su consumo, existiendo gran controversia al respecto, con estudios que encuentran beneficioso su consumo y otros que lo contraindican o limitan. En esta revisión bibliográfica se ha constatado que su consumo genera mejoras en el rendimiento físico deportivo de niños y adolescentes (mayor altura de salto y mayor fuerza isométrica manual), sin embargo, se deben tener presentes y seguir estudiando sus efectos secundarios, especialmente, por su influencia negativa sobre el sueño y las alteraciones cardiovasculares y metabólicas que desencadenan. The consumption of energy drinks has reached a great extent worldwide, especially among adolescents and children, which constitutes a real problem in terms of public health, for which renowned international organizations such as the World Health Organization or the Spanish Agency for Food Safety and Nutrition have developed various investigations in this regard. These energy drinks are characterized by having a similar composition of ingredients (i.e., caffeine, guarana, taurine, amino acids, glucuronolactone and other sugars, ginseng, group B vitamins and herbal extracts among others), although they can differ greatly in terms of dosage and amounts of each component since there is no uniform regulation on the composition, sale, promotion and distribution of these. The consumption of these drinks has been associated with benefits such as an increase in available energy, improvement in physical performance or a delay in the onset of fatigue and sleep, although it is important to point out that there is currently no uniform criterion regarding the effects associated with its consumption, with great controversy in this regard, with studies that find its consumption beneficial and others that contraindicate or limit it. In this literature review it has been found that its consumption generates improvements in the physical sports performance of children and adolescents (greater jump height and greater manual isometric strength), however, its side effects should be kept in mind and continue to be studied, especially for their negative influence on sleep and the cardiovascular and metabolic alterations they trigger. El consum de begudes energètiques o energizants ha aconseguit una gran extensió a nivell mundial, especialment entre adolescents i nens, la qual cosa constitueix una autèntica problemàtica en matèria de salut pública, per la qual cosa organismes internacionals de renom com l'Organització Mundial de la Salut o l'Agència Espanyola de Seguretat Alimentària i Nutrició han desenvolupat diverses recerques sobre aquest tema. Aquestes begudes energètiques es caracteritzen per posseir una composició similar d'ingredients (i.e., cafeïna, guaranà, taurina, aminoàcids, glucuronolactona i altres sucres, ginseng, vitamines del grup B i extractes cereals entre altres), si bé poden diferir enormement quant a dosis i quantitats de cada component ja que no existeix una regulació uniforme sobre la composició, venda, promoció i distribució d'aquestes. El consum d'aquestes begudes s'ha associat a beneficis com, increment de l'energia disponible, millora del rendiment físic o un retard en l'aparició de la fatiga i el somni, encara que és important ressenyar que actualment no hi ha un criteri uniforme quant als efectes associats al seu consum, existint gran controvèrsia sobre aquest tema, amb estudis que troben beneficiós el seu consum i altres que el contraindiquen o limiten. En aquesta revisió bibliogràfica s'ha constatat que el seu consum genera millores en el rendiment físic esportiu de nens i adolescents (major altura de salt i major força isomètrica manual), no obstant això, s'han de tenir presents i continuar estudiant els seus efectes secundaris, especialment, per la seva influència negativa sobre el somni i les alteracions cardiovasculars i metabòliques que desencadenen.

Background: Sedentary behavior (SB) is a negative routine for health, especially during advancing age. Promoting an active lifestyle and reducing SB is a global endeavor. The aim of this study was to analyze the effects of active school-based programs to interrupt prolonged sitting for daily physical activity (PA) and daily SB in children and adolescents. Methods: A systematic review was conducted. Clinical trials analyzing the effect of interrupting prolonged sitting with active breaks and classroom-based PA were included. Studies that implemented PA in class without interrupting prolonged sitting or those that implemented multimodal interventions were excluded. A systematic search was conducted in 6 databases: Medline, WOS, Cochrane Library, SPORTDiscus, CINAHL and EMBASE. Primary outcomes were daily PA and daily SB, while moderate-to vigorous physical activity (MVPA) was considered a secondary outcome. Results: Six studies were included, with a total of 976 participants between 6–14 years. The interventions were heterogeneous in duration of the sitting time interruption (5–30 min), frequency (1–3 times per-day up to three times per-week) and total duration (five days to three years). 50% of the studies scored “high risk” of bias. Three meta-analyses were performed for daily PA, MVPA and SB, showing a significant improvement in the daily PA and MVPA. Conclusions: School-based programs aimed to interrupt prolonged sitting could be a good strategy to improve daily PA and MVPA levels. Simple Summary Malnutrition and sarcopenia affect clinical outcomes in cancer patients. Nutritional evaluation in patients with neuroendocrine neoplasms (NENs) is not routinely performed. Currently, the evaluation of sarcopenia using CT scans is the gold standard in cancer patients, additionally, anthropometric, biochemical and molecular analysis of patients with gastroenteropancreatic NENs at diagnosis was perfomed. The expression levels of key ghrelin system components were assessed in 63 tumor samples. Results: Nutritional parameters were similar in GEP-NEN tumors of different origin. Relapsed disease was associated with decreased BMI. Patients who presented with weight loss at diagnosis had significantly lower overall survival (108 (25–302) vs. 263 (79–136) months). Ghrelin O-acyltransferase (GOAT) enzyme expression was higher in these patients. The prevalence of sarcopenia using CT images reached 87.2%. Mortality was observed only in patients with sarcopenia. Muscle evaluation was correlated with biochemical parameters but not with the expression of ghrelin system components. Conclusion: Survival is related to the nutritional status of patients with GEP-NENs and also to the molecular expression of some relevant ghrelin system components. Routine nutritional evaluation should be performed in these patients, in order to prescribe appropriate nutritional support, when necessary, for increasing quality of life and improving clinical outcomes. Background: Surveillance tools to estimate viral transmission dynamics in young populations are essential to guide recommendations for school opening and management during viral epidemics. Ideally, sensitive techniques are required to detect low viral load exposures among asymptomatic children. We aimed to estimate SARS-CoV-2 infection rates in children and adult populations in a school-like environment during the initial COVID-19 pandemic waves using an antibody-based field-deployable and non-invasive approach. Methods: Saliva antibody conversion defined as ≥ 4-fold increase in IgM, IgA, and/or IgG levels to five SARS-CoV-2 antigens including spike and nucleocapsid constructs was evaluated in 1509 children and 396 adults by high-throughput Luminex assays in samples collected weekly in 22 summer schools and 2 pre-schools in 27 venues in Barcelona, Spain, from June 29th to July 31st, 2020. Results: Saliva antibody conversion between two visits over a 5-week period was 3.22% (49/1518) or 2.36% if accounting for potentially cross-reactive antibodies, six times higher than the cumulative infection rate (0.53%) assessed by weekly saliva RT-PCR screening. IgG conversion was higher in adults (2.94%, 11/374) than children (1.31%, 15/1144) (p=0.035), IgG and IgA levels moderately increased with age, and antibodies were higher in females. Most antibody converters increased both IgG and IgA antibodies but some augmented either IgG or IgA, with a faster decay over time for IgA than IgG. Nucleocapsid rather than spike was the main antigen target. Anti-spike antibodies were significantly higher in individuals not reporting symptoms than symptomatic individuals, suggesting a protective role against COVID-19. Conclusion: Saliva antibody profiling including three isotypes and multiplexing antigens is a useful and user-friendlier tool for screening pediatric populations to detect low viral load exposures among children, particularly while they are not vaccinated and vulnerable to highly contagious variants, and to recommend public health policies during pandemics. IntroductionAntibiotics are commonly prescribed to young children for treating bacterial infections such as invasive pneumococcal disease (IPD) caused by Streptococcus pneumoniae. Despite the obvious benefits of antibiotics, little is known about their possible side effects on children’s nasopharyngeal microbiota. In other ecological niches, antibiotics have been described to perturb the balanced microbiota with short- and long-term effects on children’s health. The present study aims to evaluate and compare the nasopharyngeal microbiota of children with IPD and different degree of antibiotic exposure.MethodsWe investigated differences in nasopharyngeal microbiota of two groups of children ResultsMain epidemiological/clinical factors were similar across groups. Antibiotic-exposed patients were treated during a median of 4 days (IQR: 3–6) with at least one beta-lactam (100.0%). Higher bacterial richness and diversity were found in the group exposed to antibiotics. Different streptococcal amplicon sequence variants (ASVs) were differentially abundant across groups: antibiotic use was associated to lower relative abundances of Streptococcus ASV2 and Streptococcus ASV11 (phylogenetically close to S. pneumoniae), and higher relative abundances of Streptococcus ASV3 and Streptococcus ASV12 (phylogenetically close to viridans group streptococci). ASVs assigned to typical bacteria from the oral cavity, including Veillonella, Alloprevotella, Porphyromonas, Granulicatella, or Capnocytophaga, were associated to the antibiotic-exposed group. Common nosocomial genera such as Staphylococcus, Acinetobacter, and Pseudomonas were also enriched in the group exposed to antibiotics.ConclusionOur results point toward a reduction of S. pneumoniae abundance on the nasopharynx of children with IPD after antibiotic treatment and a short-term repopulation of this altered niche by oral and nosocomial bacteria. Future research studies will have to evaluate the clinical implications of these findings and if these populations would benefit from the probiotic/prebiotic administration or even from the improvement on oral hygiene practices frequently neglected among hospitalized children.

The carcinoid syndrome (CS) and hyperinsulinemic hypoglycemia (HH) represent two of the most common clinical syndromes associated with neuroendocrine neoplasms (NENs). The former is mainly related to the serotonin secretion by a small bowel NEN, whereas the latter depends on an insulin hypersecretion by a pancreatic insulinoma. Both syndromes/conditions can affect prognosis and quality of life of patients with NENs. They are often diagnosed late when patients become strongly symptomatic. Therefore, their early detection and management are a critical step in the clinical management of NEN patients. A dedicated and experienced multidisciplinary team with appropriate therapeutic strategies is needed and should be encouraged to optimize clinical outcomes. This review aims to critically analyze clinical features, evidence and treatment options of CS and HH and therefore to improve their management.

ngesta de fibra dietètica s'ha relacionat amb la salut digestiva i cardiovascular en adults, però hi ha poques evidències d'aquesta relació durant la infància. A més, les recomanacions d'ingesta de fibra en pediatria es basen en una extrapolació de les indicades per als adults, desconeixent-se si són adequades per a la salut en la infància. L’objectiu d’aquesta tesi doctoral va ser analitzar l’associació entre la ingesta de fibra dietètica i la seva adequació a les recomanacions durant la infància amb els marcadors de salut cardiometabòlics. Es va realitzar una anàlisi longitudinal observacional secundària a l'Assaig EU Childhood Obesity Project. Es va descriure la ingesta de fibra i les fonts (recollida a intervals regulars mitjançant diaris de 3 dies) des del naixement fins als 8 anys en participants d'Alemanya, Bèlgica, Polònia, Itàlia i Espanya. Els nens es van classificar segons el compliment de les recomanacions d'ingesta de fibra. Es va calcular el percentatge d'adequació a les recomanacions a totes les edats. Als 8 anys es va avaluar la seva antropometria, pressió arterial sistòlica (PAS) i les anàlisis bioquímiques en sang (lipoproteïnes, triglicèrids i homeòstasi per a resistència a la insulina (HOMA-IR)). La puntuació z interna d’aquests paràmetres es va sumar com a indicador de risc cardiometabòlic. Baixos consums de fibra de forma mantinguda al llarg de la infància es van relacionar amb menor tolerància a la glucosa, xifres superiors de PAS i una puntuació de risc cardiometabòlic més alta als 8 anys. Diferents fonts de fibra (fruites i verdures o llegums) es van associar amb diferents paràmetres del risc cardiometabòlic. Un percentatge elevat de la població infantil tenia ingestes de fibra inferiors a les recomanades de l'EFSA, encara que el fet de no complir-les no es va associar amb la salut cardiometabòlica en aquestes edats. No es va trobar un punt de tall específic d'ingesta de fibra associat a efectes beneficiosos sobre la salut als 8 anys que es pogués marcar com a nova recomanació. Tot i això, el fet que una ingesta mantinguda al voltant de 6.7 gr/1000 kcal al dia s'associés amb pitjors marcadors cardiometabòlics, podria suggerir que una ingesta superior a aquesta seria suficient per prevenir aquest tipus d'alteracions cardiometabòliques en edats primerenques. La ingesta de fibra dietética se ha relacionado con la salud digestiva y cardiovascular en adultos, pero existen escasas evidencias de esta relación durante la infancia. A los 8 años se evaluó su antropometría, presión arterial sistólica (PAS) y los análisis bioquímicos en sangre (lipoproteínas, triglicéridos y homeostasis para resistencia a la insulina (HOMA-IR)). Bajos consumos de fibra de forma mantenida a lo largo de la infancia se relacionaron con menor tolerancia a la glucosa, cifras superiores de PAS y un puntaje de riesgo cardiometabólico más alto a los 8 años. Dietary fiber intake has been associated with digestive and cardiovascular health in adults, but there is little evidence about this relationship during childhood. Furthermore, the fiber intake recommendations in pediatrics are based on an extrapolation of those indicated for adults, and it is unknown whether they are appropriate or not for children’s health. The aim of this doctoral thesis was to analyze dietary fiber intake and its adherence to recommendations in relation to cardiometabolic health markers during childhood. We performed an observational longitudinal analysis secondary to the EU Childhood Obesity Project Trial. Fiber intake and its dietary sources (collected at regular intervals using 3-day diaries) were described in participants from Germany, Belgium, Poland, Italy, and Spain from birth to 8 years. The children were classified depending on the compliance with the fiber intake recommendations. The percentage of adequacy with the recommendations was calculated at all ages. At 8 years of age, anthropometry, systolic blood pressure (SBP) and biochemical blood parameters (lipoproteins, triglycerides and homeostasis for insulin resistance (HOMA-IR)) were assessed. The internal z-score of these parameters was added as an indicator of cardiometabolic risk. Sustained low fiber intakes throughout childhood were associated with worse glucose tolerance, SBP levels, and overall cardiometabolic risk score at age 8 years. Different dietary sources of fiber (fruits and vegetables or legumes) were associated with different parameters of cardiometabolic risk. A high percentage of the child population had fiber intakes lower than those recommended by the EFSA, although not following them was not associated with cardiometabolic health at these ages. We did not find a specific cut-off point for fiber intake associated with beneficial health effects at 8 years which could be set as a new recommendation. However, the fact that an intake maintained around 6.7 g / 1000 kcal per day is associated with worse cardiometabolic markers, could suggest that an intake higher than this would be sufficient to prevent this type of cardiometabolic disturbances at an early age.

L'ús que els nens fan dels nous mitjans s'ha convertit en un fenomen molt estès al món. Per exemple, més d'una quarta part de la població mundial (1,200 milions) utilitza Internet (Tayie, 2009). A causa dels nous mitjans, el món és cada vegada més petit. Cada vegada més podem accedir a la cultura i el coneixement que solia estar més enllà del nostre horitzó. La societat de la informació d'avui dia té un gran potencial, especialment per als joves (Carlsson, 2009: 10). Aquest article aborda els resultats d'un estudi pilot qualitatiu sobre l'ús que els nens fan dels mitjans de comunicació, dut a terme entre nens egipcis d'una zona urbana i una zona rural. L'estudi és part d'un projecte comparatiu (2009-2011) sobre la participació dels joves en els mitjans (Youth Media Participation), finançat per l'Acadèmia de Finlàndia i que inclou, sobretot, a Finlàndia, Egipte, Argentina i Índia. L'objectiu principal del projecte és obtenir informació sobre l'ús que els nens fan dels nous mitjans, és a dir, l'internet i els telèfons mòbils; sobre els seus coneixements i la seva participació en els mitjans. El projecte d'investigació es va iniciar amb la recollida de dades en forma de diaris (bitàcoles) i entrevistes focals, que van ser analitzades com a material de referència per a les entrevistes futures; i dades quantitatives recollides a través de qüestionaris realitzats a la fi del 2010 als quatre països. En el present article es presenten els resultats de les entrevistes fetes a Egipte. El uso que los niños hacen de los nuevos medios de comunicación se ha convertido en un fenómeno muy extendido en el mundo. Por ejemplo, más de una cuarta parte de la población mundial (1.200 millones) utiliza internet (Tayie, 2009). Debido a los nuevos medios, el mundo es cada vez más pequeño. Cada vez más podemos acceder a la cultura y el conocimiento que solía estar más allá de nuestro horizonte. La sociedad de la información de hoy en día tiene un gran potencial, especialmente para los jóvenes (Carlsson, 2009: 10). Este artículo aborda los resultados de un estudio piloto cualitativo sobre el uso que los niños hacen de los medios de comunicación, llevado a cabo entre niños egipcios de una zona urbana y una zona rural. El estudio es parte de un proyecto comparativo (2009-2011) sobre la participación de los jóvenes en los medios de comunicación (Youth Media Participation), financiado por la Academia de Finlandia y que incluye, sobre todo, a Finlandia, Egipto, Argentina y la India. El objetivo principal del proyecto es obtener información sobre el uso que los niños hacen de los nuevos medios, es decir, internet y los teléfonos móviles; sobre sus conocimientos y su participación en los medios. El proyecto de investigación se inició con la recogida de datos en forma de diarios (bitácoras) de un día y entrevistas focales, que fueron analizadas como material de referencia para las entrevistas futuras; y datos cuantitativos recogidos a través de cuestionarios realizados a finales de 2010 en los cuatro países. En el presente artículo se presentan los resultados de las entrevistas hechas en Egipto. The use children make of the new media has become a widespread phenomenon in the world. For example, more than a quarter of the world's population (1,200 million) use the Internet (Tayie, 2009). Because of the new media, the world is becoming smaller. More and more we access the culture and knowledge that used to be beyond our horizon. Today's information society has great potential, especially for young people (Carlsson, 2009: 10).This article discusses the results of a qualitative pilot study on the use that children make of the media, conducted among Egyptian children both, in an urban and in a rural area. The study is part of a comparative project (2009-2011) on the participation of young people in the media (Youth Media Participation) funded by the Academy of Finland that includes, particularly, Finland, Egypt, Argentina and India. The main objective of the project is to obtain information on the use that children make of the new media, ie the internet and mobile phones; on their knowledge and participation in the media.The research project began with the collection of data in the form of day journals (blogs) and focus interviews, which were analyzed as reference material for future interviews, and with quantitative data collected through questionnaires completed by the end of 2010 in those four countries. This article presents the results of the interviews conducted in Egypt.

El màrqueting en jocs i advergames en línia ha estat objecte de crítiques i preocupacions. Els objectius d'aquest estudi eren tres: analitzar quin és l'estat de coneixement actual sobre els impactes del màrqueting en línia d'aliments en nens i nenes; analitzar les característiques de les tècniques de màrqueting en línia adreçades a nens i nenes en els jocs i advergames en línia més populars; i comprovar l'efecte d'un advergame que fomenta la fruita sobre el consum de fruites posterior de nens i nenes. Juntament amb aquests objectius, l'estudi també tenia com a objectiu suplir els buits de recerca actuals relacionats amb la comercialització de productes alimentaris saludables mitjançant tècniques de màrqueting en línia actualment desplegades. Els resultats no van donar cap prova que els nens que juguessin un advergame amb promoció de fruites consumissin més fruita en comparació amb aquells que jugaven un advergame amb articles no alimentaris o que estaven en estat de control. Malgrat el fet que estudis anteriors han demostrat que els anuncis que promouen els productes alimentaris augmentaven la ingesta de productes alimentaris, no es pot trobar el mateix efecte en els anuncis de promoció de fruites. Es detallen les conseqüències sobre la comercialització en línia de productes alimentaris saludables sobre les recomanacions publicitàries per a futures investigacions. El marketing en juegos y advergames en línea ha sido objeto de críticas y preocupaciones. Los objetivos de este estudio fueron triples: analizar cuál es el estado actual del conocimiento sobre los impactos del marketing de alimentos en línea para niños y niñas; analizar las características de las técnicas de marketing en línea dirigidas a niños y niñas en los juegos y advergames en línea más populares; y comprobar el efecto de un advergame que promueve la fruta en el consumo posterior de fruta de niños y niñas. Con estos objetivos, el estudio también tuvo como objetivo suplir los vacíos de investigación actuales relacionados con la comercialización de productos alimenticios saludables utilizando las técnicas de comercialización en línea actualmente implementadas. Los resultados no proporcionaron evidencia de que los niños que jugaban un advergame con promoción de fruta consumieran más fruta en comparación con los niños que jugaban un advergame con artículos no alimentarios o estaban en condiciones de control. A pesar de que estudios previos han demostrado que los advergames que promocionan productos alimenticios aumentaron la ingesta de los productos alimenticios anunciados, no se pudo encontrar el mismo efecto para los advergames que promocionan frutas. Las implicaciones en el mercadeo en línea de productos alimenticios saludables se discuten y se describen recomendaciones para futuras investigaciones. Marketing in online games and advergames has been subject to criticism and concerns. The objectives of this doctoral thesis were threefold: (1) analyse the current state of knowledge concerning the impacts of online food marketing to children; (2) analyse the characteristics of online marketing techniques directed at children in most popular online games and advergames; and (3) test the effect of an advergame that promotes fruit on children’s subsequent fruit consumption. With these objectives, the study also aimed to fill current research gaps related to the marketing of healthy food products using currently deployed online marketing techniques. The results provided no evidence that children who played an advergame promoting fruit consumed more fruit compared to those children who played an advergame with non-food items or were in the control condition. Despite the fact that previous studies have shown that advergames promoting food products increased the intake of the (often unhealthy) food products advertised, the same effect could not be found for advergames promoting fruit. Implications on online marketing of healthy food products are discussed and recommendations for future research outlined.

This paper illustrates the use of model texts as a written corrective feedback technique with young foreign language learners. The procedure used by the teacher to focus the learners' attention on grammatical, lexical and textual differences between a model story and a draft version written by the children is described, and implications are suggested for the role of feedback processing in promoting L2 learning. Aquest treball examina el paper dels textos model com a tècnica de feedback correctiu amb aprenents d'anglès a l'aula de primària. Descrivim com per mitjà de la interacció discursiva la mestra aconsegueix dirigir l'atenció dels aprenents cap a les diferències lèxiques, gramaticals i textuals entre una història model i una versió escrit pels nens. Proposem algunes implicacions de l'ús de models per a l'aprenentatge de la L2. Este trabajo examina el papel de los textos modelo como técnica de feedback correctivo con aprendices de inglés en el aula de primaria. Describimos cómo por medio de la interacción discursiva la maestra logra dirigir la atención de los aprendices hacia las diferencias léxicas, gramaticales y textuales entre una historia modelo y una versión escrito por los niños.Proponemos algunas implicaciones del uso de modelos para el aprendizaje de la L2.

La Hipercolesterolèmia Familiar (HF) és una malaltia autosòmica dominant que cursa amb nivells elevats de lipoproteïnes de baixa densitat (LDL) des del naixement. Aquesta malaltia sovint està infradiagnostica e infratractada. La infantesa és un període òptim per la detecció i per començar amb el tractament (canvis d’estil de vida (CEV) i en alguns casos, estatines). Si aquests nens són detectats i tractats de forma precoç, s’ha observat que milloren el seu pronòstic. Per aquest motiu, la present tesi doctoral té com objectiu millorar la detecció, el diagnòstic i el tractament dels nens amb HF. Aquest objectiu general s’aconsegueix mitjançant tres accions: 1. Establint una estratègia de detecció precoç per detectar nens amb HF; 2. Explorant nous biomarcadors i analitzant amb més profunditat el perfil de lipoproteïnes per ressonància magnètica nuclear (RMN); i 3. Implementant un tractament primerenc basat en activitats educatives destinades a establir CEV. Respecte al objectiu 1, gràcies a estratègies de cribratge actiu basats en la cascada directa e indirecta, amb l’ajuda dels pediatres dels centres d’atenció primària, s’han detectat 87 nens i 41 familiars amb HF en dos anys. Respecte al objectiu 2, hem observat que els nens amb HF presenten més IDOL i PCSK9, així com un major nombre de partícules LDL petites (les més aterogèniques). La Hipercolesterolemia Familiar (HF) es una enfermedad autosómica dominante que cursa con niveles elevados de lipoproteínas de baja densidad (LDL) desde el nacimiento. Esta enfermedad a menudo está infradiagnosticado e infratratada. La infancia es un periodo óptimo para la detección y para comenzar con el tratamiento (cambios de estilo de vida (CEV) y en algunos casos, estatinas). Si estos niños son detectados y tratados de forma precoz, se ha observado que mejoran su pronóstico. Por este motivo, la presente tesis doctoral tiene como objetivo mejorar la detección, el diagnóstico y el tratamiento de los niños con HF. Este objetivo general se consigue mediante tres acciones: 1. Estableciendo una estrategia de detección precoz para detectar niños con HF; 2. Explorando nuevos biomarcadores y analizando con más profundidad el perfil de lipoproteínas por resonancia magnética nuclear (RMN); y 3. Implementando un tratamiento temprano basado en actividades educativas destinadas a establecer CEV. Respecto al objetivo 1, gracias a estrategias de cribado activo basados en la cascada directa e indirecta, con la ayuda de los pediatras de los centros de atención primaria, se han detectado 87 niños y 41 familiares con HF en dos años. Respecto al objetivo 2, hemos observado que los niños con HF presentan más IDOL y PCSK9, así como un mayor número de partículas LDL pequeñas (las más aterogénicas). Familial hypercholesterolemia (FH) is an autosomal dominant disease with high levels of low-density lipoprotein (LDL) from birth. This disease is usually underdiagnosed and undertreated. Childhood is an optimal time for detection and initiation of treatment (lifestyle changes (LSC), and in some cases, statins). If these children are found and treated early, they have been shown to improve their prognosis. For this reason, the present doctoral thesis aims to improve the detection, diagnosis and treatment of children with FH. This general objective is achieved through three actions: 1. Establish an early detection strategy to detect children with FH; 2. Explore new biomarkers and analyse in greater depth the lipoprotein profile assessed by nuclear magnetic resonance (NMR); and 3. Implement early treatment based on educational activities aimed at establishing LSC. With respect to Objective 1, thanks to active screening strategies based on direct and indirect cascade, with the help of pediatricians from primary care centers, 87 children and 41 relatives with FH have been detected in two years. Regarding target 2, we observed that children with FH present more IDOL and PCSK9, as well as a greater number of small LDL particles (the most atherogenic). And finally, to answer objective 3, we study how diet affects the entire lipid and lipoprotein profile. We have observed that although elevated LDL levels in children with FH are genetically determined, LSC are capable of reducing their values.

A large part of research related to audience consumer habits focus on urban areas, not taking into account the characteristics of rural areas. The research in this paper attempts to fill this gap with a comparative study of screen use among the population aged between three and eight years old in both types of environments. One of the initial hypotheses is that children from rural areas prefer channels showing natural entertainment (interpersonal communication with peer groups, street games, playing sports, etc. ) to entertainment from virtual environments, resulting from contact with screens. In this regard, we analyze how the same sector of the population uses different resources in its leisure time and how leisure habits change according to the environment type., La mayoría de investigaciones relativas a los hábitos de consumo de la audiencia se centran en el medio urbano, sin considerar las singularidades del entorno rural. En la investigación que presentamos, tratamos de cubrir esa carencia, mediante un estudio comparativo en el consumo de pantallas de la población entre tres y dieciocho años en ambos tipos de hábitat. Una de las hipótesis de partida es que los menores de poblaciones rurales prefieren más los canales de entretenimiento natural (comunicación interpersonal con el grupo de pares, juego en la calle, práctica de deportes, etc. ) que la diversión proveniente de escenarios virtuales, fruto del contacto con pantallas. De esta manera, analizamos cómo un mismo sector de la población se abastece de diferentes recursos en su tiempo libre y de qué modo se modifican los hábitos de ocio en función del tipo de entorno.

Resumo O objetivo deste artigo é analisar os aspectos jurídicos e bioéticos da vacinação obrigatória de crianças brasileiras. Primeiro, apresentamos a vacinação como um dever decorrente do direito fundamental à saúde de crianças. Depois, realiza-se uma discussão bioética sobre o citado dever jurídico, abordando os principais problemas bioéticos envolvendo a vacinação, com destaque para os eventos adversos pós-vacinação. Concluímos que a Bioética não se propõe a estabelecer normas coercitivas, mas que ela contribui para a solução dos conflitos morais identificados em cada caso concreto ao oferecer parâmetros. Já o direito brasileiro busca assegurar o direito à saúde em sua dimensão coletiva ao estabelecer a obrigatoriedade da imunização para as crianças. Resumen El objetivo de este artículo es analizar los aspectos jurídicos y bioéticos de la vacunación de niños brasileños. En primer lugar, presentamos la vacunación como un deber derivado del derecho fundamental a la salud de los niños. Después, se realiza una discusión bioética sobre el citado deber jurídico, abordando los principales problemas bioéticos involucrando la vacunación, destacando los eventos adversos post-vacunación. Concluimos que la Bioética no se propone a establecer normas coercitivas, sino que contribuye a la solución de los conflictos morales identificados en cada caso concreto al ofrecer parámetros. El derecho brasileño busca asegurar el derecho a la salud en su dimensión colectiva al establecer la obligatoriedad de la inmunización para los niños. Abstract The objective of this article is to analyze the legal and bioethical aspects of mandatory vaccination of Brazilian children. First, we present vaccination as a duty arising from fundamental right to health of children. Afterwards, it makes a bioethical discussion about the legal duty of children's vaccination, addressing the main bioethical problems involving vaccination, especially the post-vaccination adverse events. We conclude that Bioethics does not propose to establish coercive norms, but that it contributes to the solution of the moral conflicts identified in each concrete case by offering parameters. Brazilian law seeks to ensure the right to health in its collective dimension when establishing the mandatory immunization for children. Resum L'objectiu d'aquest article és analitzar els aspectes jurídics i bioètics de la vacunació de nens brasilers. En primer lloc, presentem la vacunació com un deure derivat del dret fonamental a la salut dels nens. Després, es realitza una discussió bioètica sobre el citat deure jurídic, abordant els principals problemes bioètics involucrant la vacunació, destacant els esdeveniments adversos post-vacunació. Concloem que la Bioètica no es proposa a establir normes coercitives, sinó que contribueix a la solució dels conflictes morals identificats en cada cas concret en oferir paràmetres. El dret brasiler busca assegurar el dret a la salut en la seva dimensió col·lectiva en establir l'obligatorietat de la immunització per als nens.

INTRODUCCIÓN: Hoy en día, la fibromialgia ya es una patología conocida por la mayoría de población, para adultos se han ido encontrando formas de diagnóstico y algunos tratamientos válidos. Pero la mayoría de la población no sabe que puede presentarse en edades tempranas con diagnósticos y tratamientos mucho menos específicos. OBJETIVO: Identificar cuáles son los métodos diagnósticos y de tratamiento más utilizados para la fibromialgia juvenil, así como cuales dan mejores resultados. MÉTODOS: Se realiza una revisión sistematizada de la literatura comprendida entre los años 2010-2020 en pacientes diagnosticados de fibromialgia juvenil de entre 5-18 años. Se obtuvieron 18 estudios de las bases de datos PubMed, CINHAL y Scopus. RESULTADOS: Según la evidencia encontrada, a la hora de establecer un diagnóstico contamos con los criterios de Yunus y Masi y los criterios ACR (1990, 2010 y 2016), junto con pruebas de laboratorio normales, historial del paciente, entrevista médica y un diagnóstico diferencial con otras patologías, son los parámetros más utilizados. Respecto al tratamiento, se obtienen los mejores resultados de TCC junto con ejercicio y educación sobre la fibromialgia, pero en ningún caso una curación total o lo suficientemente buena. El tratamiento farmacológico es utilizado en última instancia y tampoco cuenta con validación de ningún fármaco concreto. Esto resultados pueden deberse, en gran parte, a la dificultad de realizar estudios con pacientes jóvenes. CONCLUSIONES: La fibromialgia juvenil sigue siendo un campo poco explorado y controvertido, en el que todavía no se ha establecido ningún resultado validado tanto para diagnostico como para tratamiento. Però la majoria de la població no sap que puede presentar-se en edats primerenques amb diagnòstics i tractaments molt menys específics. OBJECTIU: Identificar quins són els mètodes diagnòstics i de tractament més utilitzats per a la fibromiàlgia juvenil, així com quals donen millors resultats. MÈTODES: Es realitza una revisió sistematitzada de la literatura compresa entre els anys 2010-2020 en pacients diagnosticats de fibromiàlgia juvenil d'entre 5-18 anys. Es van obtenir 18 estudis de les bases de dades PubMed, CINHAL i Scopus. RESULTATS: Segons l'evidència trobada, a l'hora d'establir un diagnòstic comptem amb els criteris de Yunus i Masi i els criteris ACR (1990, 2010 i 2016), juntament amb proves de laboratori normals, historial de l'pacient, entrevista mèdica i un diagnòstic diferencial amb altres patologies, són els paràmetres més utilitzats. Pel que fa a el tractament, s'obtenen els millors resultats de TCC juntament amb exercici i educació sobre la fibromiàlgia, però en cap cas una curació total o prou bona. El tractament farmacològic és utilitzat en última instància i tampoc compte amb validació de cap fàrmac concret. Aquests resultats poden deure’s, en gran part, a la dificultat de realitzar estudis amb pacients joves. CONCLUSIONS: La fibromiàlgia juvenil continua sent un camp poc explorat i controvertit, en el qual encara no s'ha establert cap resultat validat tant per a diagnostic com per a tractament. INTRODUCTION: Nowadays, fibromyalgia is already a pathology known by the majority of the population. For adults, diagnostic forms and some valid treatment have been found. But the majority of the population does not know that it can occur at an early age with much less specific diagnoses and treatments. OBJECTIVE: To identify which are the most used diagnostic and treatment methods for juvenile fibromyalgia, as well as which give better results. METHODS: A systematic review of the literature between the years 2010-2020 is carried out in patients diagnosed with juvenile fibromyalgia aged 5-18 years. Eighteen studies were obtained from the PubMed, CINHAL and Scopus databases. RESULTS: According to the evidence found, when establishing a diagnosis we have the Yunus and Masi criteria and the ACR criteria (1990, 2010 and 2016), together with normal laboratory tests, patient history, medical interview and a differential diagnosis with other pathologies are the most used parameters. Regarding treatment, the best results of CBT are obtained together with exercise and education about fibromyalgia, but in no case a total cure or good enough. Pharmacological treatment is used as a last resort and does not have validation of any specific drug. These results may be due, in large part, to the difficulty of conducting studies with young patients. CONCLUSIONS: Juvenile fibromyalgia continues to be a little explored and controversial field, in which no validated result has yet been established for both diagnosis and treatment.
