Introduction and Objectives: Autoimmune hepatitis (AIH) is a rare disease with a complex and not fully understood pathogenesis. Prognostic factors that might influence treatment response, relapse rates, and transplant-free survival are not well established. This study investigates clinical and biochemical markers associated with response to immunosuppression in patients with AIH. Materials and Methods: This retrospective cohort study included 102 patients with AIH treated with immunosuppressants and followed at the Federal University of Minas Gerais, Brazil, from 1990 to 2018. Pretreatment data such as clinical profiles, laboratory, and histological exams were analyzed regarding biochemical response at one year, histological remission, relapse, and death/transplantation rates. Results: Cirrhosis was present in 59 % of cases at diagnosis. One-year biochemical remission was observed in 55.7 % of the patients and was found to be a protective factor for liver transplant. Overall survival was 89 %. Patients with ascites at disease onset showed a higher aspartate aminotransferase (AST)/ alanine aminotransferase (ALT) ratio and elevated Model of end-stage liver disease (MELD) score. The presence of ascites was significantly associated with a 20-fold increase in mortality rate. Conclusions: AIH has a severe clinical phenotype in Brazilians, with high rates of cirrhosis and low remission rates. Early diagnosis and treatment are essential for achieving remission and reducing complications. The presence of ascites is significantly associated with mortality, emphasizing the importance of monitoring and prompt intervention. This study also stresses the need for further research on AIH in Latin America.

ABSTRACT Although lung cancer (LC) is one of the most common and lethal tumors, only 15% of patients are diagnosed at an early stage. Smoking is still responsible for more than 85% of cases. Lung cancer screening (LCS) with low-dose CT (LDCT) reduces LC-related mortality by 20%, and that reduction reaches 38% when LCS by LDCT is combined with smoking cessation. In the last decade, a number of countries have adopted population-based LCS as a public health recommendation. Albeit still incipient, discussion on this topic in Brazil is becoming increasingly broad and necessary. With the aim of increasing knowledge and stimulating debate on LCS, the Brazilian Society of Thoracic Surgery, the Brazilian Thoracic Association, and the Brazilian College of Radiology and Diagnostic Imaging convened a panel of experts to prepare recommendations for LCS in Brazil. The recommendations presented here were based on a narrative review of the literature, with an emphasis on large population-based studies, systematic reviews, and the recommendations of international guidelines, and were developed after extensive discussion by the panel of experts. The following topics were reviewed: reasons for screening; general considerations about smoking; epidemiology of LC; eligibility criteria; incidental findings; granulomatous lesions; probabilistic models; minimum requirements for LDCT; volumetric acquisition; risks of screening; minimum structure and role of the multidisciplinary team; practice according to the Lung CT Screening Reporting and Data System; costs versus benefits of screening; and future perspectives for LCS.

Abstract Background Health promotion and disease prevention programme registries (HPPRs), also called ‘best practice portals’, serve as entry points and practical repositories that provide decision-makers with easy access to (evidence-based) practices. However, there is limited knowledge of differences or overlaps of howe current national HPPRs in Europe function, the context and circumstances in which these HPPRs were developed, and the mechanisms utilised by each HPPR for the assessment, classification and quality improvement of the included practices. This study prepared an overview of different approaches in several national HPPRs and the EU Best Practice Portal (EU BPP) as well as identified commonalities and differences among the core characteristics of the HPPRs. Methods We conducted a descriptive comparison – that focused on six European countries with existing or recently developed/implemented national HPPR and the EU BPP –to create a comparative overview. We used coding mechanisms to identify commonalities and differences; we performed data management, collection and building consensus during EuroHealthNet Thematic Working Group meetings. Results All HPPRs offer a broad range of health promotion and disease-prevention practices and serve to support practitioners, policymakers and researchers in selecting practices. Almost all HPPRs have an assessment process in place or planned, requiring the application of assessment criteria that differ among the HPPRs. While all HPPRs collect and share recommendable practices, others have implemented further measures to improve the quality of the submitted practices. Different dissemination tools and strategies are employed to promote the use of the HPPRs, including social media, newsletters and publications as well as capacity building workshops for practice owners or technical options to connect citizens/patients with local practices. Conclusions Collaboration between HPPRs (at national and EU level) is appreciated, especially regarding the use consistent terminology to avoid misinterpretation, facilitate cross-country comparison and enable discussions on the adaption of assessment criteria by national HPPRs. Greater efforts are needed to promote the actual implementation and transfer of practices at the national level to address public health challenges with proven and effective practices.

Introdução: O tratamento de doenças raras tem sido um desafio para o Sistema Único de Saúde brasileiro. Além dos altos custos dos tratamentos, as características inerentes a esse tipo de doença trazem fragilidades para as evidências científicas de eficácia e segurança. A Comissão Nacional de Incorporação de Tecnologias no Sistema Único de Saúde (Conitec) é formada por três Comitês de especialistas: Medicamentos, Produtos e procedimentos, Protocolos clínicos e diretrizes terapêuticas para que avaliam mensalmente as demandas e emitam parecer sobre incorporação, exclusão ou alteração das tecnologias no Sistema Único de Saúde (SUS). Objetivos: Avaliar dados qualitativos e quantitativos sobre os critérios considerados pelos tomadores de decisão em saúde da Conitec e analisar quais doenças raras foram beneficiadas pelas recomendações da comissão. Material e Método: Este é um estudo exploratório, descritivo e retrospectivo. Os dados de junho de 2012 a junho de 2023 foram coletados do site da Conitec por meio de um formulário de extração específico e analisados utilizando estatísticas descritivas. Resultados: Um total de 790 tecnologias foram avaliadas de junho de 2012 a junho de 2023, sendo 164 medicamentos para doenças raras. Dentre esses, as doenças que mais se beneficiaram foram esclerose múltipla com 13,3% (n=21), fibrose cística com 6,3% (n=10) e hipertensão pulmonar com 5,7% (n=9). Dentre as tecnologias incorporadas ao Sistema Único de Saúde para o tratamento de doenças raras, 27 tiveram inicialmente uma recomendação desfavorável e somente após consulta pública foram recomendadas para incorporação. Os motivos que contribuíram para essa mudança na recomendação foram a nova evidência científica apresentada (64%), nova negociação do preço do medicamento (28%) e novo impacto orçamentário pela revisão do cálculo da população-alvo (8%). Discussão e Conclusões: Os critérios para avaliar tecnologias para doenças raras são semelhantes aos adotados para outras condições clínicas. No entanto, é importante adotar critérios específicos na análise de medicamentos destinados a doenças consideradas raras para a população visando ampliar a equidade e o acesso a novos tratamentos. A negociação de preços com a indústria é um fator importante que foi destacado, favorecendo potencialmente o acesso a novos tratamentos que podem modificar a história natural dessas doenças com melhora clínica dos pacientes.

Introdução: A Comissão Nacional de Incorporação de Tecnologias no Sistema Único de Saúde (Conitec), instituída em 2011 por meio de lei federal, por meio do processo de avaliação de tecnologias, busca embasar suas recomendações com as melhores evidências científicas disponíveis sobre a eficácia, acurácia, efetividade e segurança das tecnologias analisadas, além de considerar um estudo de avaliação econômica comparativa e análise de impacto orçamentário. Objetivos: Identificar medicamentos excluídos e quais os critérios e abordagens utilizadas pela Conitec para recomendar essas exclusões ao SUS. Material e Método: Foi realizado um estudo exploratório, descritivo e retrospectivo para analisar os critérios considerados pela Conitec na exclusão de medicamentos do SUS entre junho de 2012 e junho de 2023. Os dados foram coletados no site do Conitec e organizados em um formulário específico para extração, sendo posteriormente analisados por meio de estatística descritiva. Resultados: Durante o período de avaliação, foram identificadas um total de 790 recomendações de tecnologias pela Conitec, das quais 75 (9,4%) estavam relacionadas à exclusão de medicamentos. Ao analisar as recomendações de exclusão, uma série de fatores foram considerados, que incluíam atualizações de protocolos, medicamentos com prazo de validade expirado ou sem registro na Agência Nacional de Vigilância Sanitária, avaliação de tecnologias mais custo-efetivas que aquelas já disponíveis no SUS, além de medicamentos cuja indicação clínica não estava incluída na lista dos fármacos registrados para tratamentos no sistema de saúde (Relação Nacional de Medicamentos Essenciais – Rename). Discussão e Conclusões: Embora a padronização dos critérios para a exclusão de tecnologias em saúde ainda esteja em processo de estabelecimentompela Conitec, este estudo fornece insights valiosos para compreendermos as recomendações de exclusão de medicamentos no sistema de saúde brasileiro. As recomendações identificadas sugerem um potencial impacto positivo na população, ao permitir uma alocação mais eficiente dos recursos do sistema de saúde. Além disso, a exclusão de determinadas terapias parece contribuir para a prevenção de possíveis riscos de interações medicamentosas e reações adversas indesejáveis, aumentando a segurança dos pacientes. No entanto, é importante reconhecer a necessidade contínua de aprimorar os critérios de exclusão, visando maior transparência e coerência nas recomendações da Conitec.

Introdução: A Comissão Nacional de Incorporação de Tecnologias no Sistema Único de Saúde (Conitec) é responsável por definir os critérios e avaliar a incorporação de novas tecnologias no sistema de saúde brasileiro. Com o objetivo de promover transparência e garantir a participação pública, a Conitec adota um processo de consulta pública, no qual as recomendações preliminares são abertas à contribuição da sociedade. Essas consultas públicas desempenham um papel crucial ao permitir que diversos atores, como profissionais de saúde, pacientes, indústria farmacêutica e sociedade em geral, expressem suas opiniões, experiências e conhecimentos, enriquecendo assim o processo de avaliação de tecnologias em saúde e promovendo decisões mais informadas e legitimadas. Objetivos: Extrair dados qualitativos e quantitativos sobre as recomendações em saúde da Conitec e identificar quais alterações ocorreram após a submissão dos relatórios à consulta pública (CP). Material e Método: Trata-se de um estudo descritivo, retrospectivo e exploratório de dados coletados do sítio eletrônico da Conitec, entre junho de 2012 a junho de 2023. Foi utilizado um formulário específico de extração de dados, que por sua vez foram analisados por meio de estatística descritiva. Resultados: Medicamentos representaram o tipo de tecnologia mais avaliado (77%), seguidos por procedimentos (15%) e produtos de saúde (8%). Durante o período, foram analisadas um total de 790 recomendações. Dessas, 70% permaneceram inalteradas entre o relatório preliminar e o final publicado, 9% sofreram modificações e 20% foram análises simplificadas de tecnologias de interesse público relevante, de baixo custo e impacto orçamentário reduzido. Essas análises não foram submetidas à consulta ou audiência pública. No relatório final, a maioria das recomendações (45%) foi modificada, especialmente aquelas relacionadas a condições clínicas da área de genética (14%), todas sendo condições clínicas raras, seguidas por oncologia (14%) e neurologia (12%). A maior proporção de alterações nas recomendações preliminares está relacionada à apresentação de novas evidências clínicas (51%), seguida por propostas de preço alternativas (38%), por parte de propostas das empresas farmacêuticas. Discussão e Conclusões: Significativas alterações nas recomendações da Conitec ocorreram devido à apresentação de novas evidências clínicas e propostas de preços alternativas, durante a consulta pública. Em um cenário de constantes avanços tecnológicos e restrições orçamentárias, é importante a atuação flexível da Conitec ao considerar novas informações e propostas que viabilizem o acesso a tecnologias eficazes. Esse estudo demonstra que as consultas públicas desempenham um papel crucial na avaliação e incorporação de tecnologias ao Sistema Único de Saúde.

ABSTRACT Objective: To investigate the impact of tenofovir disoproxil fumarate on bone mineral density and bone mineral content in children and adolescents infected with the human immunodeficiency virus. Data source: The search procedure was performed according to the Preferred Reporting Items for Systematic Reviews and Meta-Analyses Statement. The search was carried out until April 2022 in Medical Literature Analysis and Retrieval System Online (Medline), Embase, Cochrane Central, Latin American and Caribbean Health Sciences Literature, Web of Science, Scopus, and MedRxiv. The combination of terms used was: (Children OR Youth OR Teenagers) AND HIV AND (Tenofovir OR “Antiretroviral therapy”) AND (“Bone density” OR Osteoporosis OR Osteopenia). The protocol was registered in the International Prospective Register of Systematic Reviews (PROSPERO, CRD42022312851) Data synthesis: The initial searches resulted in 1156 papers. After the exclusion of duplicate studies, three blinded reviewers analyzed the title and abstract of 563 papers, of which 57 remained to be read in full. Only nine papers met the eligibility criteria and were included in descriptive and risk-of-bias analyses. Regarding study design, four were cross-sectional, three were longitudinal before-after studies without a control group, and two were prospective cohorts. Among these nine papers, seven showed no significant association between tenofovir disoproxil fumarate use and reduced bone mass in young people. However, these papers did not have high methodological quality. Conclusions: Although most of the selected papers found no harmful effect of tenofovir disoproxil fumarate on bone mass, further primary research with higher methodological quality is needed so robust scientific evidences can be obtained.

ABSTRACT This cross-sectional study aimed to evaluate the autonomic response of older women in the six-minute walk test. In total, 32 women aged 60 years or older without a diagnosed health problem were evaluated during the six-minute walk test. To monitor the autonomic response, the following variables were considered: heart rate, systolic and diastolic blood pressure, respiratory rate, and perceived exertion. These variables were compared during rest, effort, and recovery. This study also sought a correlation between autonomic function variables and performance in the test and perceived exertion. Results showed that the effort made by older women in the six-minute walk test induces an autonomic response resulting in increased heart rate and systolic and diastolic blood pressure; however, the respiratory rate remained unchanged during the test. Diastolic blood pressure remained high during recovery. No correlation was found neither between perceived exertion and cardiovascular physiological response nor between distance covered and variation of the autonomic response or level of physical conditioning. Therefore, the effort spent in the six-minute walk test promotes an autonomic response in older women, increasing cardiovascular stress without increasing ventilation. In this context, the Borg scale was not representative of cardiovascular stress during the test.

Introduction and objectives: Primary biliary cholangitis (PBC) and autoimmune hepatitis (AIH) and PBC overlap syndrome (AIH/PBC) have been associated with a higher risk of hepatocellular carcinoma (HCC) and extra-hepatic malignancy (EHM). This study aims to assess potential risk factors associated with cancer development in PBC and AIH/PBC. Materials and methods: The Brazilian Cholestasis Study Group database was reviewed to compare clinical and laboratory features of PBC patients with HCC and EHM with those without cancer. Results: Among the 752 PBC patients enrolled, 64 of them with AIH/PBC, 87 cancers were identified in 72 patients, including 20 cases of HCC and 67 of EHM. Patients with HCC had a higher prevalence of cirrhosis (95% vs. 32.5% of those subjects without cancer, p≤0.001), smoking (55% vs. 12.3%, p≤0.001), CREST syndrome (30% vs 7.6%, p=0.003) and prior azathioprine (30% vs 8%, p= 0.005) and prednisone (35% vs 14%, p= 0.018) use, whereas patients with EHM had a higher prevalence of smoking (42.3% vs 12.4% of those subjects without cancer, p=

Introdução: Medicamentos são produtos farmacêuticos tecnicamente obtidos ou elaborados, com finalidade profilática, curativa, paliativa ou para fins de diagnóstico. A dificuldade de acesso a medicamentos pode acarretar em comprometimento da segurança do processo assistencial. Objetivo: apresentar o cenário brasileiro de gastos com medicamento entre os anos de 2015 e 2019, sob a ótica das Contas SHA. Método: Trata-se de um estudo descritivo, a partir da análise de dados secundários, tendo como unidade de investigação as informações sobre gastos em medicamentos no Brasil do setor público e privado. Resultado: Os gastos totais em saúde foram, em média, R$ 620,2 bilhões por ano no período de 2015 a 2019 o que representa, em média, cerca de 9% do Produto Interno Bruto (PIB) do período. Do valor total referente aos gastos em saúde, cerca de 18% são gastos anualmente em medicamentos. O desembolso das famílias representou, em média, 89,3% dos gastos, enquanto a porcentagem do governo ficou em 10,7%, em média. Os dados apontam uma redução da participação governamental no período, contudo, o Governo Federal é o maior responsável pelos desembolsos de recursos para a aquisição de medicamentos e artigos médicos não duráveis no Brasil. Já os gastos com medicamentos do Programa Farmácia Popular do Brasil somaram, em média, R$ 2,6 bilhões, embora tenha havido uma redução de R$ 800 milhões na série analisada. Conclusão: O fortalecimento e ampliação das políticas públicas de assistência farmacêutica são essenciais para subsidiar a população, ampliando o acesso a medicamentos e reduzindo o comprometimento da renda das famílias.

Increasing epidemiological evidence suggests a bidirectional relationship between non-alcoholic fatty liver disease (NAFLD) and type 2 diabetes, and that NAFLD may precede and/or promote the development of diabetes. This study aimed to investigate whether liver steatosis is associated with the incidence of diabetes in the Brazilian Longitudinal Study of Adult Health (ELSA-Brasil). The ELSA-Brasil is an occupational cohort study of active or retired civil servants, aged 35-74 years, in six capital cities in Brazil. We excluded participants with diabetes at baseline, those who reported excessive alcohol consumption or with missing information on relevant covariates, and those with self-referred hepatitis or cirrhosis. In total, 8,166 individuals participated, and the mean duration of follow-up was 3.8 years. The Cox proportional regression model was used to estimate the adjusted hazard ratio (HR) for the associations. Abdominal ultrasonography was used to detect liver steatosis. In the follow-up period, the cumulative incidence of diabetes was 5.25% in the whole sample, 7.83% and 3.88% in the groups with and without hepatic steatosis, respectively (p < 0.001). Compared to those without steatosis, individuals with hepatic steatosis had an increased risk of developing diabetes (HR = 1.31; 95%CI: 1.09-1.56) after adjustment for potential confounders, including body mass index (BMI). Hepatic steatosis was an independent predictor of incident diabetes in the ELSA-Brasil cohort study. Physicians should encourage changes in lifestyle and screen for diabetes in patients with fatty liver.

ABSTRACT BACKGROUND: Occult hepatitis B virus infection (OBI) is defined as the presence of hepatitis B virus (HBV) deoxyribonucleic acid (DNA) in the liver of individuals with undetectable hepatitis B virus surface antigen (HBsAg) in the serum. The actual prevalence of OBI and its clinical relevance are not yet fully understood. OBJECTIVE: To evaluate the prevalence of HBV DNA in liver biopsies of HBsAg-negative patients with chronic liver disease of different etiologies in a referral center in Brazil and compare two different HBV DNA amplification protocols to detect HBV. DESIGN AND SETTING: This cross-sectional observational study was conducted at the Liver Outpatient Clinic, Hospital das Clínicas, Universidade Federal de Minas Gerais, Belo Horizonte, MG, Brazil, between January 2016 and December 2019. METHODS: HBV DNA was investigated in 104 liver biopsy samples from individuals with chronic liver disease of different etiologies, in whom HBsAg was undetectable in serum by nested-polymerase chain reaction (nested-PCR), using two different protocols. RESULTS: OBI, diagnosed by detecting HBV DNA using both protocols, was detected in 6.7% of the 104 individuals investigated. Both protocols showed a good reliability. CONCLUSION: In addition to the differences in the prevalence of HBV infection in different regions, variations in the polymerase chain reaction technique used for HBV DNA amplification may be responsible for the large variations in the prevalence of OBI identified in different studies. There is a need for better standardization of the diagnostic methods used to diagnose this entity.

Streptomyces sp. BRA-346 is an Actinobacteria isolated from the Brazilian endemic tunicate Euherdmania sp. We have reported that this strain produces epoxyketone peptides, as dihydroeponemycin (DHE) and structurally related analogs. This cocktail of epoxyketone peptides inhibits the proteasome chymotrypsin-like activity and shows high cytotoxicity to glioma cells. However, low yields and poor reproducibility of epoxyketone peptides production by BRA-346 under laboratory cultivation have limited the isolation of epoxyketone peptides for additional studies. Here, we evaluated several cultivation methods using different culture media and chemical elicitors to increase the repertoire of peptide epoxyketone production by this bacterium. Furthermore, BRA-346 genome was sequenced, revealing its broad genetic potential, which is mostly hidden under laboratory conditions. By using specific growth conditions, we were able to evidence different classes of secondary metabolites produced by BRA-346. In addition, by combining genome mining with untargeted metabolomics, we could link the metabolites produced by BRA-346 to its genetic capacity and potential regulators. A single biosynthetic gene cluster (BGC) was related to the production of the target epoxyketone peptides by BRA-346. The candidate BGC displays conserved biosynthetic enzymes with the reported eponemycin (EPN) and TMC-86A (TMC) BGCs. The core of the putative epoxyketone peptide BGC (ORFs A-L), in which ORF A is a LuxR-like transcription factor, was cloned into a heterologous host. The recombinant organism was capable to produce TMC and EPN natural products, along with the biosynthetic intermediates DH-TMC and DHE, and additional congeners. A phylogenetic analysis of the epn/tmc BGC revealed related BGCs in public databases. Most of them carry a proteasome beta-subunit, however, lacking an assigned specialized metabolite. The retrieved BGCs also display a diversity of regulatory genes and TTA codons, indicating tight regulation of this BGC at the transcription and translational levels. These results demonstrate the plasticity of the epn/tmc BGC of BRA-346 in producing epoxyketone peptides and the feasibility of their production in a heterologous host. This work also highlights the capacity of BRA-346 to tightly regulate its secondary metabolism and shed light on how to awake silent gene clusters of Streptomyces sp. BRA-346 to allow the production of pharmacologically important biosynthetic products.

Em território latino-americano, a chamada “guerra contra as drogas” tem se apresentado como importante moduladora das relações de opressão que determinam hierarquias de raça, classe e gênero na região. Catalisadores de processos que remontam ao colonialismo, os delitos de drogas têm servido como legitimadores da manutenção de uma ordem de extermínio e encarceramento que atinge preferencialmente pessoas negras e, cada vez mais, mulheres. Através de revisão bibliográfica, este artigo pretende debater como a política está revolvida por constructos de ódio e medo próprios; constitui práticas de criminalização de pessoas negras e pobres em território marginal; bem como os impactos específicos no encarceramento de mulheres neste território.

Aim: Up to 40% of patients with primary biliary cholangitis (PBC) will have a suboptimal biochemical response to ursodeoxycholic acid (UDCA), which can be improved by the addition of fibrates. This exploratory study aims to evaluate the long-term real-life biochemical response of different fibrates, including ciprofibrate, in subjects with UDCA-unresponsive PBC.Methods: The Brazilian Cholestasis Study Group multicenter database was reviewed to assess the response rates to UDCA plus fibrates in patients with UDCA-unresponsive PBC 1 and 2 years after treatment initiation by different validated criteria.Results: In total, 27 patients (100% women, mean age 48.9 ± 9.2 years) with PBC were included. Overall response rates to fibrates by each validated criterion varied from 39 to 60% and 39–76% at 12 and 24 months after treatment combination, respectively. Combination therapy resulted in a significant decrease in ALT and ALP only after 2 years, while GGT significantly improved in the first year of treatment. Treatment response rates at 1 and 2 years appear to be comparable between ciprofibrate and bezafibrate using all available criteria.Conclusion: Our findings endorse the efficacy of fibrate add-on treatment in PBC patients with suboptimal response to UDCA. Ciprofibrate appears to be at least as effective as bezafibrate and should be assessed in large clinical trials as a possibly new, cheaper, and promising option for treatment of UDCA-unresponsive PBC patients.

Introduction and objectives: Little is known about primary biliary cholangitis (PBC) in non-whites. The purpose of this study was to evaluate clinical features and outcomes of PBC in a highly admixed population. Material and methods: The Brazilian Cholestasis Study Group multicentre database was reviewed to assess demographics, clinical features and treatment outcomes of Brazilian patients with PBC. Results: 562 patients (95% females, mean age 51 ± 11 years) with PBC were included. Concurrent autoimmune diseases and overlap with autoimmune hepatitis (AIH) occurred, respectively, in 18.9% and 14%. After a mean follow-up was 6.2 ± 5.3 years, 32% had cirrhosis, 7% underwent liver transplantation and 3% died of liver-related causes. 96% were treated with ursodeoxycholic acid (UDCA) and 12% required add-on therapy with fibrates, either bezafibrate, fenofibrate or ciprofibrate. Response to UDCA and to UDCA/fibrates therapy varied from 39%-67% and 42-61%, respectively, according to different validated criteria. Advanced histological stages and non-adherence to treatment were associated with primary non-response to UDCA, while lower baseline alkaline phosphatase (ALP) and aspartate aminotransferase (AST) levels correlated with better responses to both UDCA and UDCA/fibrates.Conclusions: Clinical features of PBC in highly admixed Brazilians were similar to those reported in Caucasians and Asians, but with inferior rates of overlap syndrome with AIH. Response to UDCA was lower than expected and inversely associated with histological stage and baseline AST and ALP levels. Most of patients benefited from add-on fibrates, including ciprofibrate. A huge heterogeneity in response to UDCA therapy according to available international criteria was observed and reinforces the need of global standardization.

Background: No previous study have evaluated transient elastography (TE) for predicting esophageal varices (EV) in hepatosplenic schistosomiasis (HSS). Aim: To investigate noninvasive methods of predicting EV in patients with HSS mansoni. Methods: Cross-sectional multicentric study included 51 patients with HSS. Patients underwent ultrasonography-dopplerfluxometry, upper endoscopy, complete blood cell count and TE (Fibroscan®) for liver and spleen stiffness measurement (LSM and SSM). Noninvasive scores previously established for cirrhotic population were studied: platelet count to spleen diameter ratio (PSR), LSM-spleen diameter to platelet ratio score (LSPS) and varices risk score (VRS). We proposed a version of LSPS and VRS by replacing LSM with SSM and named them SSPS and modified-VRS, respectively. Results: EV was detected in 42 (82.4%) subjects. Individuals with EV presented higher SSM (73.5 vs 36.3 Kpa, p=0.001), splenic vein diameter (10.8 vs 8.0 mm, p=0.017), SSPS (18.7 vs 6.7, p=0.003) and modified-VRS (4.0 vs 1.4, p=0.013), besides lower PSR (332 vs 542, p=0.038), than those without EV. SSPS was independently associated with EV presence (OR=1.19, 95%CI 1.03-1.37, p=0.020) after multivariate analysis. In a model excluding noninvasive scores, SSM was independently associated with EV diagnosis (OR=1.09, 95%CI 1.03-1.16, p=0.004). AUROC was 0.856 (95%CI 0.752-0.961, p=0.001) for SSM and 0.816 (95%CI 0.699-0.932, p=0.003) for SSPS (p=0.551). Conclusions: Spleen-related variables were predictors of EV: SSM, splenic vein diameter, SSPS, modified-VRS and PSR. Multivariate models indicated that SSM and SSPS are useful tools for predicting EV in non-cirrhotic portal hypertension by HSS and may be used in clinical practice.

Background: The interaction between lean body mass (LBM) and fat mass index (FMI) with grip strength (GS) has not been explored in the same analysis model in adolescents. This study thus aims to analyze the association between FMI and LBM with GS. Methods: This cross-sectional study was conducted with data from the 2016 follow-up of the 1997/98 Birth Cohort of São Luís. Grip strength was assessed by the Jamar Plus + dynamometer. The LBM and FMI indexes were assessed [ratio of the mass (lean or fat-kg) to height (m2)]. The confounding variables identified for the relationship between FMI and LBM with GS in the same analysis model, by directed acyclic graph (DAG), were sex, age, race, work, alcohol consumption, smoking, physical activity, and consumption of ultra-processed foods and culinary preparations, used in the adjusted analysis. Results: A total of 2339 adolescents (52.5% girls) were analyzed. The boys have a higher GS than the girls. In the adjusted analysis, with each increase of 1 kg/m2 in the FMI, GS was reduced by 0.72 kgf for boys and 0.35 kgf for girls. At each increase of 1 kg/m2 in the LBM, GS increased by 2.18 kgf for boys and 1.26 kgf for girls. Conclusions: FMI was associated with lower GS regardless of the LBM. LBM was associated with higher GS regardless of the FMI.

Introduction and objectives: Little is known about the etiology of acute liver failure (ALF) in Latin America. The objective of this paper is to investigate the main etiologies of ALF in Brazil, including Drug Induced Liver Injury (DILI) using stringent causality criteria. Patients or material and methods: All the cases of individuals who underwent liver transplantation (LT) in 12 centers in Brazil for ALF were reviewed. When DILI was stated as the cause of ALF, causality criteria were applied on site by the main investigator in order to rule out other etiologies. Results: 325 individuals had ALF mainly for unknown reasons (34%), DILI (27%) and AIH (18%). Reassessment of the 89 cases of DILI, using stringent causality criteria, revealed that in only 42 subjects could DILI be confirmed as the cause of ALF. Acetaminophen (APAP) toxicity (n = 3) or DILI due to herbal and dietary supplements (HDS) (n = 2) were not commonly observed. Conclusions: Undetermined etiology and DILI are the main causes of ALF in Brazil. However, APAP toxicity and DILI due to HDS are mostly uncommon.

The development of an organism is a controlled process, which can be disrupted by genetic or environmental stress. Although fluctuating asymmetry is widely used as an indicator of developmental instability, its effectiveness has been questioned due to the contradictory results produced by this technique which, at least in part, probably reflects methodological inappropriateness. Here, we investigated if wing asymmetry of drosophilids increases when they develop during the dry season in the Brazilian savanna, considered a stressful season for these insects. Using protocols designed to avoid methodological problems, we analysed the wings of Zaprionus indianus and three species of the genus Drosophila (D. mercatorum, D. simulans, and D. sturtevanti). There was no significative difference in wing asymmetry in any of the four species between the dry and rainy seasons. The similar wing asymmetry levels between seasons may mean that during the dry season drosophilids are submitted to strong natural selection and the asymmetric individuals have less chance of surviving. Alternatively, environmental drought may not affect the wing symmetry. Although our study added more data to the relationship between asymmetry and stress, this discussion seems to be far from being solved.

Background. Primary sclerosing cholangitis (PSC) is associated with a broad phenotypic spectrum in different populations from diverse ethnic and racial backgrounds. This study aimed to describe the clinical characteristics and outcomes of PSC in a multicenter cohort of patients from Brazil. Methods. Data from the Brazilian Cholestasis Study Group were retrospectively reviewed to assess demographic information and clinical characteristics of PSC, as well as the outcomes, such as transplantation-free survival. Results. This cohort included 210 patients. After excluding 33 (15.7%) patients with PSC and overlap syndrome of autoimmune hepatitis, 177 (97 males, median age 33 (21–42) years) with clear-cut PSC were eligible for this study. Most of the patients (n = 139, 78.5%) were symptomatic, and 104 (58.7%) had advanced PSC at the time of diagnosis. Concurrent inflammatory bowel disease was observed in 78 (58.6%) of the investigated patients (n = 133), and most of them had ulcerative colitis (n = 61, 78.2%). The 1- and 5-year survival free of liver transplantation or death were 92.3 ± 2.1% and 66.9 ± 4.2%, respectively, and baseline advanced PSC, pruritus, and elevated bilirubin levels were independent risk factors for the composite adverse outcome. Females were significantly older and had lower bilirubin levels than males at baseline, but survival was not associated with sex. Approximately 12.4% (n = 22) of patients with PSC died, and 32.8% (n = 58) underwent liver transplantation at a median follow-up time of 5.3 and 3.2 years. Conclusion. Multiethnic Brazilian PSC patients exhibited a less pronounced male predominance and a lower frequency of inflammatory bowel disease than Caucasians. Adverse outcomes were more frequent, probably due to advanced disease at baseline.

A partir de uma análise jurisprudencial da Corte Constitucional italiana identificam-se os elementos que, atendendo à igualdade formal, estruturam critérios objetivos e razoáveis para que seja atribuído o efeito vinculante às suas decisões.

Background & Aims: The precise determination of non-alcoholic fatty liver disease (NAFLD) onset is challenging. Thus, the initial hepatic responses to fat accumulation, which may be fundamental to our understanding of NAFLD evolution and clinical outcomes, are largely unknown. Herein, we chronologically mapped the immunologic and metabolic changes in the liver during the early stages of fatty liver disease in mice and compared this with human NAFLD samples. Methods: Liver biopsies from patients with NAFLD (NAFLD activity score [NAS] 2–3) were collected for gene expression profiling. Mice received a high-fat diet for short periods to mimic initial steatosis and the hepatic immune response was investigated using a combination of confocal intravital imaging, gene expression, cell isolation, flow cytometry and bone marrow transplantation assays. Results: We observed major immunologic changes in patients with NAS 2–3 and in mice in the initial stages of NAFLD. In mice, these changes significantly increased mortality rates upon drug-induced liver injury, as well as predisposing mice to bacterial infections. Moreover, deletion of Toll-like receptor 4 in liver cells dampened tolerogenesis, particularly in Kupffer cells, in the initial stages of dietary insult. Conclusion: The hepatic immune system acts as a sentinel for early and minor changes in hepatic lipid content, mounting a biphasic response upon dietary insult. Priming of liver immune cells by gut-derived Toll-like receptor 4 ligands plays an important role in liver tolerance in initial phases, but continuous exposure to insults may lead to damage and reduced ability to control infections. Lay summary: Fatty liver is a very common form of hepatic disease, leading to millions of cases of cirrhosis every year. Patients are often asymptomatic until becoming very sick. Therefore, it is important that we expand our knowledge of the early stages of disease pathogenesis, to enable early diagnosis. Herein, we show that even in the early stages of fatty liver disease, there are significant alterations in genes involved in the inflammatory response, suggesting that the hepatic immune system is disturbed even following minor and undetectable changes in liver fat content. This could have implications for the diagnosis and clinical management of fatty liver disease.

Resorting to a One Strain Many Compounds (OSMAC) approach, the marine Streptomyces sp. BRB081 strain was grown in six different media settings over 1, 2, 3 or 7 days. Extractions of mycelium and broth were conducted separately for each media and cultivation period by sonication using methanol/acetone 1:1 and agitation with ethyl acetate, respectively. All methanol/acetone and ethyl acetate crude extracts were analysed by HPLC-MS/MS and data treatment was performed through GNPS platform using MZmine 2 software. In parallel, the genome was sequenced, assembled and mined to search for biosynthetic gene clusters (BGC) of secondary metabolites using the AntiSMASH 5.0 software. Spectral library search tool allowed the annotation of desferrioxamines, fatty acid amides, diketopiperazines, xanthurenic acid and, remarkably, the cyclic octapeptides surugamides. Molecular network analysis allowed the observation of the surugamides cluster, where surugamide A and the protonated molecule corresponding to the B-E isomers, as well as two potentially new analogues, were detected. Data treatment through MZmine 2 software allowed to distinguish that the largest amount of surugamides was obtained by cultivating BRB081 in SCB medium during 7 days and extraction of culture broth. Using the same data treatment, a chemical barcode was created for easy visualization and comparison of the metabolites produced overtime in all media. By genome mining of BRB081 four regions of biosynthetic gene clusters of secondary metabolites were detected supporting the metabolic data. Cytotoxic evaluation of all crude extracts using MTT assay revealed the highest bioactivity was also observed for extracts obtained in the optimal conditions as those for surugamides production, suggesting these to be the main active compounds herein. This method allowed the identification of compounds in the crude extracts and guided the selection of best conditions for production of bioactive compounds.

Cidades brasileiras enfrentam diariamente dificuldades ocasionadas pelos congestionamentos, com perdas significativas na qualidade de vida da população. São vários os problemas que conduzem a esse estado de degradação. Entre eles, destaca-se, nos sistemas de transporte urbano de passageiros, a falta de capacidade dos diversos modos do transporte público em ofertar lugares de forma compatível com a demanda. Este artigo propõe realizar uma análise socioeconômica dos ganhos com a redução dos congestionamentos, da poluição, dos tempos de viagens, dos acidentes e do consumo de combustível, a partir da introdução de um modo de maior capacidade e consumidor de uma forma de energia menos poluente. Além disso, e principalmente, pretende analisar a possibilidade de se viabilizar o financiamento dos projetos metroferroviários por meio da economia do consumo de combustíveis derivados do petróleo. Essa viabilização se daria com a substituição do diesel consumido no rodoviário pelo consumo de eletricidade no metroferroviário. A avaliação foi aplicada para o projeto do metrô em Curitiba. Os resultados mostram que os ganhos socioeconômicos podem financiar grande parte do empreendimento e apresentar ganhos na qualidade de vida, com a redução dos níveis de poluentes do ar, dos acidentes de trânsito e dos tempos de deslocamento dos habitantes de Curitiba.

O trabalho tem como objetivo inicial demonstrar os caminhos percorridos pelos sindicatos, desde o Estado Liberal – com a formação do Direito do Trabalho –, até o Estado Social – na fase de consolidação do ramo trabalhista. Em sequência, investigar-se-á a consagração da liberdade sindical como direito humano e sua relação com os parâmetros democráticos e pluralistas do Estado Democrático de Direito. Essa correlação permitiria, ao inserir os trabalhadores – e suas organizações sindicais – nos centros de poder, não apenas efetivar o princípio, mas repensar o papel dos sindicatos, agora que fundamentalizados e atrelados aos próprios paradigmas do Estado.

The herpes simplex virus type 2 (HSV-2) is a sexually transmitted infection prevalent in the world; in spite of despite diagnostic and therapeutic advances. According to estimates in 2012, it was calculated that there were 417 million people infected worldwide. The objective of this project was to detect HSV-2 in indigenous women of the Zulia state, for that 100 women of the Wayúu and Añú ethnic groups, were selected without discriminating age and/or parity. The physician proceeded to sample with an alginate swab, for the processing of the same was used the polymerase chain reaction (PCR) method to detect HSV-2 in samples of cervical-uterine origin, and cytological diagnosis was made through staining with hematoxylin-eosin. 22% of HSV-2 positive samples were obtained, the results show that a considerable percentage of patients present HSV-2 infection, which is extremely important because it can contribute to the progression of processes inflammatory agents.

Este trabalho analisa a possibilidade de adoção conjunta por núcleos familiares formados por pessoas do mesmo sexo, considerando a decisão do STF na ADPF 132 e da ADI 4277, que atribui a essas uniões os efeitos jurídicos da união estável. Pretende-se apresentar o instituto da adoção no Brasil a partir da concepção pluralista de família e buscar compreender o alcance da decisão do STF, debatendo alguns votos considerados controvertidos.

In developing countries, cardiovascular risk factors are poorly controlled, leading to high prevalence of cerebrovascular diseases. The aim of the study was to evaluate the burden of white matter lesions in magnetic resonance through the Fazekas scale in a population aged 75+ years living in the community, and to investigate possible associations between vascular lesions, cardiovascular risk factors and cognitive status. Subjects were selected from a community-based study on brain aging conducted in Caeté (Minas Gerais state), Brazil. Overall, 177 participants (112 cognitively healthy, 36 with cognitive impairment-no dementia and 29 with dementia), being 108 women, aged 79.3 ± 3.8 years, with 3.1 ± 2.9 years of educational level, underwent a 3 Tesla magnetic resonance scanner with fluid attenuated image recovery acquisition. Severity of white matter lesions was assessed through the Fazekas scale. Severe white matter lesions were present in 31.1% of the whole sample and in 25.0% of the cognitively healthy individuals. A significant association was found between severe white matter lesions and cognitive impairment (OR = 2.20, 95% CI 1.17–6.53; p = 0.021), as well as with hypertension (OR = 1.92, 95% CI 1.03–7.39; p = 0.043). In conclusion, a high prevalence of severe white matter lesions was observed in this elderly Brazilian population sample, and white matter lesions were associated with hypertension and cognitive status. Importantly, the prevalence of white matter lesions was also high in cognitively healthy subjects. Keywords: White matter lesions, Aging, Dementia, Hypertension

This article analyzes the Institute of Legitimation of Possession, as a form of public land acquisition in the State of Pará and investigates the applicable regulations, since the State Law of Para n. 7.289 / 2009 established the legitimacy as a mean of public land alienation, without expressly mentioning the State Decree no. 1054/1996, which declared the forfeiture of the possession of records made before the state land agencies, whose areas were not legitimized.

Neste trabalho analisou-se como o sistema de ensino superior em uma região do Brasil contribui com processos de dominação na sociedade, dificultando a mobilidade social de indivíduos oriundos de classes desfavorecidas. Para a análise teórica, foram utilizados os conceitos de campo, habitus e dominação de Bourdieu e a teoria institucional. Os dados foram coletados por meio de entrevistas semiestruturadas com representantes de empresas de Recursos Humanos (RH), gestores de recursos humanos de grandes empresas e seis coordenadores de cursos das IESs da região de Feira de Santana, Bahia, e usou-se a análise de conteúdo para a sua interpretação (BARDIN, 2011). Os resultados apontam que há uma consciência sobre as limitações dos alunos em razão dos processos que envolvem a posse de capital (econômico, cultural e social) e as suas condições de existência. Além disso, há uma aceitação consciente das diferenças detectadas entre as chances de seus alunos e aqueles oriundos de IESs mais bem avaliadas pelo mercado, o que indica que dificilmente as IESs mencionadas conseguirão oferecer aos seus alunos nível de competitividade necessário para o sucesso profissional. Conclui-se, também, que as IESs têm sido muito mais mantenedoras da estabilidade e de posições das classes sociais do que promotoras de mudança nessas posições. Um resultado importante é mostrar que as diferenças entre IESs da área estudada e de outras regiões podem ser explicadas pelo processo de dominação no campo educacional e que os atores do campo analisado aceitam essa situação. Este artigo avalia, na percepção dos alunos de Instituições de Ensino Superior(IESs) do curso de Administração de uma região no estado da Bahia, os fatores que explicam suas escolhas e práticas ao longo de sua jornada acadêmica. Para a compreensão deste fenômeno, foram utilizados os conceitos de habitus e capital (BOURDIEU 1983) e teoria institucional (MEYER e ROWAN, 1991). A partir de questionários aplicados em 6 IESs, no período entre fevereiro e junho de 2014, foram obtidas 527 respostas de alunos do primeiro ao oitavo semestre. Os dados foram analisados a partir de análise fatorial e regressão múltipla. Os resultados indicam que fatores como docentes, família, capital social e as práticas da IES são correlacionadas com as escolhas e práticas dos alunos durante o curso, especialmente o seu grau de envolvimento com os estudos, o que pode afetar suas chances de inserção profissional no futuro.

O trabalho tem como objetivo construir propostas para a superação do sistema confederativo inflexível por categorias que, em última análise, constitui um dos pilares do enrijecimento sindical brasileiro. Partindo da análise da agregação sindical, pretende-se identificar os sentidos corporativos do instituto para, posteriormente, delinear os fatores que conduziram à sua inadequação interpretativa após a promulgação da Constituição de 1988. Por fim, a partir da lógica transicional de organização sindical – lógica inclusive absorvida, em termos, pela própria CLT – confrontam-se os dogmas enrijecedores do sindicalismo brasileiro e as premissas interpretativas delineadas pela Constituição de 1988, na busca por uma readequação do critério de agregação – de impositivo para flexível. Propõe-se, portanto, um novo caminho para o enfrentamento da crise de identidade e organização que acomete o sindicalismo brasileiro, permitindo uma reconfiguração interna e externa do sindicato para que possa reassumir seu papel histórico de ator – e não apenas de mero espectador – na construção, desenvolvimento e afirmação do Direito do Trabalho.

O trabalho apresenta de forma sucinta algumas características do denominado constitucionalismo latino-americano, que se destaca pela tentativa de privilegiar a riqueza cultural e as tradições comunitárias e históricas na busca de uma refundação das instituições políticas e jurídicas. Esse movimento constitucionalista, como se pretende mostrar, ao incluir em sua agenda questões de cunho ambiental, social, econômico e jurídico tem acendido esperanças de que o desenvolvimento sustentável, fulcrado em uma nova visão do mundo, seria possível. A partir dessa visão, o artigo apresenta a noção do Sumak Kawsay (expressão de uma forma ancestral de ser e de estar no mundo) adotado pelo Plan Nacional para el Buen Vivir do Equador e por outros países como Bolívia e Venezuela, que propõe uma ruptura dos conceitos ortodoxos do desenvolvimento de hoje (crescimento, rapidez, exportação, dentre outros). O trabalho destaca a visão da natureza e do mundo expostas pelo Sumak Kawsay, como uma possível alternativa ao desenvolvimento sustentável. Aborda, ainda que de forma breve, algumas ideias do ecossocialismo e sua possível correlação com o Sumak Kawsay.

The study addresses the public budget as an instrument that evidences a series of choices guided by public policies, revealing that its development should at least mirror an arena of political struggle, to provide that the divergent social forces move to achieve their interests. The article also mentions the Portuguese experience of off-budget as an administrative decentralization process and the studies of Charles Taylor concerning the desired structuring of new meanings for human actions that can travel between the individual issues and public space in search of a recognition process. It is proposed to build a democratic model of budget planning associated with a vision to provide the participation of all recipients in order to pursue an ethic that allows individuals to recover what gives them the possibility of designin g existentially meaningful ways of life and engagement in society.

Functional properties of substances present in in natura foods such as fruits and vegetables are well documented; however, the activity that remains after processing needs more research. The present study aimed to evaluate the antioxidant potential in fruit processed as sauce and quantify the compounds able to contribute to such activity. Three different treatments were developed varying only the concentration of pepper Capsicum frutescens L., with treatment ratios (fruit: water: vinegar: salt) being: treatment 1 (0.5: 1: 0.5: 0.33), 2 (1: 1: 0.5: 0.33), and 3 (2: 1: 0.5: 0.33). By the DPPH method, the values found for EC50 (g g DPPH−1) from 3726.9 to 5425.9 for the alcoholic extract were the most significant. The content of total phenols did not vary between the three treatments. While the content of carotenoids found was significantly different in the treatment with lower content of the fruit in natura, when compared to the treatment with higher content (44.02 and 56.09 μg of β-carotene 100 g−1, respectively) and the content of ascorbic acid varied between 10.95 and 21.59 mg 100−1 g. Therefore, the pepper sauce was presented as an alternative to the consumption of bioactive compounds that may have antioxidant potential.

Objetivou-se avaliar o desempenho produtivo de genótipos de trigo com diferentes qualidades industriais em resposta às proporções de parcelamento da adubação nitrogenada em cobertura. Foram avaliados seis genótipos de trigo, sendo estes: IPR 136 e LD 052114 (melhorador), IPR 130 e IPR 144 (pão) e BRS Louro e CD 105 (básico). A adubação nitrogenada em cobertura foi realizada na dose fixa de 80 kg ha-1 na forma de ureia, com cinco formas de parcelamento como segue: P1- dose total aplicada no perfilhamento aos 20 DAE (dias após a emergência); P2 - ¼ no emborrachamento e o restante aos 20 DAE; P3 - ½ aos 20 DAE + ½ no emborrachamento; P4 - ¼ aos 20 DAE e o restante no emborrachamento e P5 - dose total de N aplicada no emborrachamento. As proporções de parcelamento da adubação nitrogenada em cobertura não afetaram o ciclo, o grau de acamamento, o peso hectolítrico (PH), a massa de 1.000 grãos, o número de espiguetas por espiga, o número de grãos por espigueta, o número de grãos por espiga, a massa de grãos por espiga e o nº de espiga/m². Os genótipos IPR 136 e LD 052114 apresentaram maiores rendimentos de grãos quando o N foi aplicado em uma única vez em P1 e P5, respectivamente. Para os genótipos BRS Louro, IPR 130 e CD 105 as diferentes épocas de adubação nitrogenada não afetaram a produtividade de grãos. Na cultivar IPR 144, a adubação nitrogenada em cobertura dos tratamentos P1, P2 e P5 favoreceu a produtividade de grãos.This study aimed to evaluate the performance of wheat genotypes of different industrial qualities in response to the ratios of nitrogen (N) fertilisation topdressing. Six genotypes of wheat were evaluated, those being: IPR LD 136 and 052 114 (breeder), IPR 130 and IPR 144 (bread) and BRS Louro and CD 105 (basic). The nitrogen fertilisation topdressing was applied at a fixed dosage of 80 kg ha-1 as urea, divided five ways: P1 - the total dosage being applied when tilling, at 20 DAE (days after emergence); P2 - ¼ when booting and the remainder at 20 DAE; P3 - ½ at 20 DAE + ½ at booting; P4 - ¼ at 20 DAE and the remainder at booting; P5 - total N dosage applied at booting. The proportions for splitting the nitrogen fertilisation topdressing did not affect the cycle, the degree of bedding, the weight per hectolitre (HW), the 1000 grain mass, the number of spikelets per spike, the number of grains per spikelet, the number of grains per spike, the grain mass per spike and the number of spikes/m². The genotypes IPR 136 and 052 114 LD had higher grain yields when N was applied as a single application in P1 and P5 respectively. For the genotypes BRS Louro, IPR 130 and CD 105, the different nitrogen fertilization times did not affect grain yield. In cultivar IPR 144, the nitrogen fertilization topdressing in treatments P1, P2 and P5 favoured grain yield.

O texto tem por objeto traçar o perfil da famí­lia na pós-modernidade, sublinhando o papel de função e de promoção que desempenha, na busca do fomento ao livre desenvolvimento de seus membros. O estudo analisa também algu­mas decisões do Superior Tribunal de Justiça que, ao fundamento da infidelidade – conside­rada óbice para a configuração da união estável –, negam proteção jurídica a núcleos familiares paralelos. O trabalho apresenta a noção de pânico moral, o que possivelmente explicaria o conservadorismo de algumas instituições. Debate-se, outrossim, o conceito de monoga­mia como princípio estruturante do Direito de Família, insinuando que a família, na condição de núcleo de peculiaridade dinâmica, pode assumir variados contornos. Como grupo de pessoas em união estável, voluntária e coope­rativa, que desempenha a função de promover e proteger seus integrantes, a família não há de ser havida apenas como elemento posto pelo legislador, resultando, também, de opções nas relações privadas, que podem transcender ao modelo dado pelo legislador.

No presente trabalho objetivou-se elaborar e avaliar as características físico-químicas e aceitabilidade de licores de casca de tangerina, variedade Ponkan, com graduação alcoólica de 18ºGL e teor de açúcar de 350g.L-1, produzidos com diferentes concentrações de casca e tempos de processamento. Os tratamentos testados foram: 250g de casca e 7 dias de processamento, 250g de casca e 14 dias de processamento, 500g de casca e 7 dias de processamento e 500g de casca e 14 dias de processamento. Nos licores as análises realizadas foram: teor alcoólico, densidade, pH, sólidos solúveis, acidez titulável e teste de aceitabilidade. Na análise estatística o delineamento experimental utilizado foi o inteiramente casualizado (DIC), em esquema fatorial 2 x 2 com o uso do teste de Tukey na comparação das mé- dias dos tratamentos a 1 e 5% de probabilidade. Para o teste de aceitabilidade também foi aplicado o teste de Tukey a 1 e 5% de probabilidade. Nas análises foram utilizadas 6 repetições. Para o teste de aceitabilidade foram utilizados 100 provadores não treinados. O licor correspondente ao tratamento com 500g de casca e 14 dias de processamento, foi o que apresentou melhor comportamento quanto às suas características físico-químicas avaliadas. Todos os licores tiveram boa aceitação sensorial, obtendo médias acima de 7 (gostei regularmente). O licor preparado com a maior concentração de casca e maior tempo de processamento (500g de casca e 14 dias de processamento) foi um dos mais aceitos quanto à cor, aroma e textura, embora o mesmo não tenha ocorrido com o quesito sabor.

Este trabalho teve como objetivo processar e avaliar a qualidade e conservação de bananas-passa cv. Prata em rodelas secas em diferentes temperaturas e armazenadas a temperatura ambiente. Bananas cv. Prata, foram amadurecidas ao natural (até grau 7 de coloração da casca), lavadas, sanificadas com solução de cloro livre 200ppm por 5 min, descascadas, fatiadas em rodelas e secas em diferentes temperaturas (65°C, 75°C e 85°C) por 6 horas. Após 24 horas foram embaladas em sacos de polietileno de alta densidade transparentes e armazenadas a temperatura ambiente 28±2°C e 58±4%UR. A cada 30 dias foram realizadas análises físico-químicas, químicas, microbioló- gicas e teste de aceitabilidade, até os produtos estarem fora dos padrões estabelecidos pela legislação. O delineamento experimental utilizado foi o inteiramente casualizado, em esquema de parcelas subdivididas no tempo com o uso do teste de Scott-Knott na comparação das médias dos tratamentos a 5% de probabilidade. Para o teste de aceitabilidade foi aplicado o teste não paramétrico de Friedman a 5% de probabilidade. Foram utilizadas 3 repetições, com a unidade experimental constituída de um saco de polietileno contendo 80g de bananas-passa. Para as características físico-químicas, diferenças foram detectadas com relação ao pH, sendo que bananas secas a 65°C e 85o C apresentaram redução de pH até os 103 e 107 dias, respectivamente, com elevação dos valores até o final do armazenamento. Na temperatura de 75o C houve decréscimo linear no pH até os 150 dias. Com relação aos sólidos solúveis, bananas secas a 85o C, apresentaram menor teor aos 92 dias com elevação dos teores até o final do armazenamento. Quanto à avalia- ção microbiológica, todas as temperaturas apresentaram valores abaixo dos parâmetros exigidos pela legislação. O teste de aceitabilidade mostrou a preferência dos consumidores pelas bananas secas a 85°C nos quesitos cor, aroma e sabor e pelas bananas secas a 65°C no quesito textura. Bananas secas a 65°C, 75°C e 85°C podem ser armazenadas em sacos de polietileno de alta densidade transparente e permanecerem aptas para o consumo até 150 dias de armazenamento.

INTRODUCTION: Neoadjuvant chemoradiation promotes tumor size reduction and staging before the surgery, reducing the risk of involving the circumferential resection margin and local recurrence. For patients who have been submitted to the neoadjuvant therapy, the usefulness of a second nuclear magnetic resonance (MRI) after chemoradiation has not been clearly explained. Objective: Assess the degree of tumor regression and downstaging after chemoradiation using MRI, compared with the pathology, and its correlation with surgical outcomes and patient prognosis. METHODS: This study investigated 13 patients. Their mean age was 52.3 years and 69.23% were male. RESULTS: The agreement in T and N staging was 30.76%, between the second MRI and pathology, overestimated in 55.55% of the remaining. T staging agreement was 53.84% and N staging agreement, 61.53%. The circumferential resection margin was free of cancer in 100%. The survival rate was 92%, with 75% disease-free in a mean follow-up of 1-2 years. CONCLUSION: A second MRI after chemoradiation can evaluate the degree of tumor regression, but with low compliance in relation to pathology, with tendency to overstaging. More studies are required to confirm these initial observations.INTRODUÇÃO: A radioquimioterapia neoadjuvante promove redução do tamanho e do estadiamento dos tumores do reto antes da cirurgia, reduzindo o risco de acometimento de margem de ressecção circunferencial e da recorrência local. Para pacientes que se submeteram a neoadjuvância, a realização de uma segunda ressonância magnética (RNM) após a radioquimioterapia, para avaliação do resultado do tratamento, pode trazer dados relevantes para a programação cirúrgica e previsão do prognóstico, porém sua utilização ainda é controversa. OBJETIVO: Avaliar a capacidade da RNM prever o grau de regressão tumoral e o downstaging obtidos e a correlação entre o grau de regressão tumoral com o prognóstico dos pacientes. Métodos: Foram incluídos 13 pacientes até o momento; desses 69,23% eram do sexo masculino e a idade média foi de 52,3 anos. RESULTADOS: O anatomopatológico (AP) mostrou conformidade em relação ao estadiamento T e N estimado pela RNM pós-neoadjuvância de 30,76%; nos demais pacientes, houve tendência ao superestadiamento em 55,55%. No estadiamento T houve concordância de 53,84% e quanto ao status linfonodal, concordância 61,53%. A margem de ressecção circunferencial foi livre de neoplasia em 100%. A sobrevida foi de 92%, com 75% de sobrevida livre de doença num seguimento médio de 1-2 anos. CONCLUSÃO: Uma segunda ressonância após neoadjuvância pode avaliar se houve regressão tumoral, porém com baixa conformidade em relação ao anatomopatológico, com tendência ao superestadiamento. Mais estudos são necessários para corroborar essas impressões iniciais.

OBJETIVOS: Avaliar os efeitos da manobra hiperinsuflação manual com compressão torácica em termos de depuração de secreções, mecânica pulmonar, hemodinâmica e oxigenação em pacientes sob ventilação mecânica invasiva. MÉTODOS: Foi realizado estudo controlado do tipo cruzado, com vinte pacientes com mais de 48 horas em ventilação mecânica invasiva. Após 4 horas da última aspiração os pacientes foram submetidos aos procedimentos, aspiração ou hiperinsuflação manual com compressão torácica e aspiração, um após o outro, respeitando intervalo de quatro horas, em ordem estabelecida conforme randomização. As variáveis foram coletadas nos momentos pré, durante e após 5, 15, 30 e 60 minutos do término dos procedimentos. A secreção aspirada foi coletada e mensurada. RESULTADOS: Não foram encontradas alterações significativas para o volume corrente, pressão platô e complacência após os dois procedimentos durante o estudo (p>0,05). As variáveis hemodinâmicas apresentaram comportamento distinto ao longo do tempo caracterizado por aumento das pressões e frequência durante a realização dos procedimentos, com retorno aos valores basais após 5 minutos das intervenções (p≤0,001). Não foi encontrada diferença no comportamento hemodinâmico entre os procedimentos (p>0,05). A saturação de oxigênio durante todos os momentos do estudo foi 99%, com exceção de dois momentos durante a realização da HMCT+ASP que foi 98% (pOBJECTIVES: To evaluate the effects of the manual hyperinflation with thoracic compression (MHTC) maneuver on the clearance of secretions, pulmonary mechanics, hemodynamics and oxygenation in mechanically ventilated patients. METHODS: This was a controlled, crossover study that included twenty patients who were under invasive ventilation for more than 48 hours. Four hours after the last airway suctioning procedure, the patients underwent the study interventions, Suction alone or MHTC plus Suction, in sequence at four hour intervals. The sequence order for the procedures was established by randomization. Data were collected before, during and 5, 15, 30 and 60 minutes after each intervention. The suctioned secretions were collected and weighed. RESULTS: No significant differences between the procedures were found regarding tidal volume, plateau pressure and pulmonary compliance (p>0.05). The hemodynamic variables showed increased pressures and heart rate during the procedures and returned to baseline values five minutes after the end of the procedure (p≤0.001). No significant hemodynamic differences were seen between the interventions (p>0.05). Este trabalho teve como objetivo avaliar a aplicação de diferentes concentrações de ácido ascórbico em melões ‘Orange Flesh’ minimamente processados. Melões inteiros foram sanifi cados com 500mg L-1 de hipoclorito de sódio por 10 minutos e os cortes em cubos com 100mg L-1 por 1 minuto antes de serem tratados com as concentrações de ácido ascórbico (0, 1, 2 e 3%) em imersão a temperatura ambiente por 10 minutos. Após drenagem, os melões em cubos foram acondicionados em embalagens PET recobertas com polietileno de 18µm e armazenados a 5±1°C e 85±5% de UR por 8 dias, sendo avaliados a cada 2 dias. Os produtos foram submetidos a análises físicoquímicas, microbiológicas e sensoriais. O delineamento experimental utilizado foi o inteiramente casualizado em esquema fatorial com a comparação das médias através do Teste de Tukey. Nas análises não destrutivas foram utilizadas 10 repetições e nas destrutivas 3 repetições. A aplica- ção de ácido ascórbico reduziu a perda de massa; os frutos apresentaram baixas populações de bactérias psicrotrófi cas, fungos fi lamentosos e leveduras, redução dos sólidos solúveis, do pH, da fi rmeza, e consequentemente estendeu a vida útil pós-colheita em 2 dias. A aparência, o aroma e o sabor também foram afetados. A aplicação de 1% de ácido ascórbico foi o melhor tratamento para melões “Orange Flesh” minimamente processado.

The objective of this study was to identify promising wheat genotypes from the IAPAR breeding program for crossesby multivariate techniques. Measures of dissimilarity between genotypes were obtained from multicategoric variables. To identifyclusters, Tocher optimization and UPGMA were used. The genotypes were divided into five diverging groups, indicating a widegenetic base of the wheat breeding program of the Agricultural Research Institute of Paraná (IAPAR). As indicated by the geneticdistance, crosses involving the cultivars IPR 85 and IPR 118 seem to be the most promising to increase genetic gains in both grainand flour quality as well as for agronomic parameters.

Viral hepatitis is a public health problem worldwide, caused by different etiologic agents, such as hepatitis A, B, C, D, E, F or G. However, cases have been reported related to liver disease caused by hepatotropic viruses, such as cytomegalovirus (CMV) and Epstein Barr Virus (EBV). The aim of this study was to establish the differential diagnosis of hepatotropic and non-hepatotropic viruses in patients with symptoms and signs suggestive of liver infection in the city of Maracaibo, Venezuela, 2007. 268 blood samples were collected from patients at different health centers in the city of Maracaibo, who recurred to the Regional Viral Reference Laboratory. The presence of IgM antibodies to hepatitis A virus (HAV), B (HBV), C (HCV), CMV and EBV was detected by ELISA. The results indicate a high incidence of HAV 118/268 (44.03%), followed by HBV 5/268 (1.86%) and the absence of HCV. For non-hepatotropic viruses, 4.48% (12/268) were positive for EBV and 9.7% (26/268) for CMV. For the above-mentioned viruses, 39.92% (107/268) are considered negative. This study suggests that CMV and EBV viruses are often involved with liver damage and should be considered as possible etiologic agents for any case of hepatitis.

A azaleia é uma planta ornamental propagada comercialmente por meio de estacas, sendo difícil o seu o enraizamento em muitos casos, o que resulta em baixa produção de mudas. O objetivo do trabalho foi avaliar o uso do AIB e o efeito de diferentes substratos sobre o enraizamento de estacas herbáceas de azaleia no outono. O delineamento experimental foi o inteiramente casualizado, com cinco repetições, em um fatorial 2x3, sendo adotados os fatores: aplicação ou não de ácido indol butírico - AIB (0mg L-1 e 1.000mg L-1) e tipos de substratos (vermiculita - grânulos médios, casca de arroz carbonizada e fibra de coco). Após 76 dias, foram avaliadas as seguintes variáveis: retenção foliar (%), estacas enraizadas (%), sobrevivência das estacas (%), comprimento da maior raiz, número de folhas, massa de matéria fresca das raízes por estaca, pH e condutividade elétrica dos substratos. Os resultados mostraram não haver diferenças para o enraizamento pela aplicação ou não de AIB. As estacas enraizadas em casca de arroz carbonizada e vermiculita apresentaram número superior de folhas em relação às estacas do substrato fibra de coco. No entanto, a fibra de coco e a casca de arroz carbonizada proporcionaram um maior comprimento de raiz. Para massa de matéria fresca por estaca, a fibra de coco também apresentou o melhor resultado. Não houve contribuição do AIB no enraizamento de estacas de azaleia, e o substrato fibra de coco foi o que apresentou melhor desempenho para o enraizamento das estacas.Azalea is an ornamental plant commercially propagated by cuttings, and their rooting process in many cases, is difficult, which results in decreased production of seedlings. The objective of this study was to evaluate the use of IBA and the effect of different substrates on the rooting of cuttings of azalea in the fall. A randomized design with five replications in a factorial array was used as a statistical model, and it was adopted the following factors: types of substrates (vermiculite - medium granules, hull rice coal and coconut fiber), and indole butyric acid - IBA (0mg L-1 and 1,000mg L-1). After 76 days, the following variables were evaluated: leaf retention (%), rooted cuttings (%), cuttings survival (%), length of the longest root, number of leaves, fresh weight of roots per cutting, pH and electrical conductivity of the substrates. There was no difference of using IBA to improve rooting. The hull rice coal and vermiculite substrates resulted in a higher number of leaves on cuttings than coconut fiber. However, the coconut fiber and hull rice coal provided a greater root length. For fresh weight of roots per cutting, the coconut fiber also provided the best results. It was concluded that there was no contribution of IBA on the rooting of azalea, and coconut fiber substrate was more suitable for its rooting.
