Objective The objective of this study was to characterize the independent risk factors for seizures in critically ill patients monitored with continuous EEG (cEEG). Methods We retrospectively investigated variables associated with cEEG seizures, first in the entire cohort of 156 patients and, subsequently, in the subgroup without seizures in the first 30 minutes of monitoring. Results Seizures were observed in 19.2% of recordings, and in 50% of these, seizures occurred in the first 30 minutes. In the entire cohort, epilepsy, acute seizures prior to cEEG, interictal epileptiform discharges (IEDs), lateralized periodic discharges (LPDs), and brief potentially ictal rhythmic discharges (BIRDs) were associated with a higher incidence of cEEG seizures, whereas coma, intravenous anaesthetic drugs, and generalized periodic discharges (GPDs) were associated with a lower incidence of seizures. On multivariate analysis, this association was maintained for acute seizures before cEEG (OR: 5.92) and IEDs (OR: 6.81). Excluding patients with seizures at the beginning of monitoring, acute seizures before cEEG, IEDs, LPDs, and BIRDs were associated with an increased risk of seizures. The presence of IEDs or LPDs in the first 30 minutes was associated with a 4.14-fold greater chance of seizures on cEEG. On multivariate analysis, acute seizures prior to recording (OR 7.29) and LPDs (OR: 5.38) remained associated with seizures on cEEG. Due to the sample size, BIRDs were not included in multivariate models. Significance Acute seizures prior to monitoring, IEDs, LPDs and BIRDs are important risk factors for cEEG seizures in critically ill patients.

Epileptic encephalopathies are conditions in which the abundant epileptiform activity itself interferes with development, resulting in cognitive slowing and often regression, psychiatric and behavioral disturbances. Nonconvulsive status epilepticus has been defined as ongoing or nonrecovering nonconvulsive seizures. It has been challenging to differentiate clinical and electroencephalographic patterns in epileptic encephalopathies from those attributed to nonconvulsive status epilepticus, since several different epileptic encephalopathies may show continuous or subcontinuous epileptiform activity. Especially for patients with known epileptic encephalopathy, the new proposal for diagnosis of nonconvulsive status epilepticus suggests an increase in prominence or frequency of specific electroencephalographic features as compared to baseline correlated to clinical and EEG responsiveness to intravenous anti-seizure drugs. This clinical change may be unclear, particularly in patients with pre-existent cognitive or behavioral impairments. This review intends to organize previously published data, with available information in the literature on some of those specific epileptic syndromes and diseases, focusing on two main questions: i. When should specialists suspect of nonconvulsive status epilepticus in epileptic encephalopathies? ii. Could epileptic encephalopathies themselves be nonconvulsive status epilepticus presentations? Lastly, the rationale for definition and treatment in many of the epileptic encephalopathies is the effect of ongoing frequent epileptiform activity on development and cognition, and this will require monitoring with serial clinical, neurophysiological, functional neuroimaging, and neuropsychological assessments. Whether there would be an association or causality between epileptic encephalopathies and nonconvulsive status epilepticus is a key question demanding further research.

Self-limited epilepsy with centrotemporal spikes, formerly called benign epilepsy with centrotemporal spikes, or rolandic epilepsy, is an age-related and well-defined epileptic syndrome. Since seizures associated with rolandic spikes are infrequent and usually occur during sleep, and repetitive or prolonged EEG recording for diagnostic purposes is not necessary for diagnosis, reports of ictal video-electroencephalographic seizures in this syndrome are rare. We aimed to show ictal video-EEG of typical rolandic seizures. We report the ictal video-EEG recordings of two children with rolandic epilepsy who presented typical rolandic seizures during routine recording. Case 1: A 9-year-old boy, with normal development, had his first seizure at 8 years old, characterized by paresthesia in his left face, blocking of speech, and drooling. Carbamazepine was started with seizure control. Case 2: A 10-year-old boy, with normal development, started with focal seizures during sleep, characterized by eye and perioral deviation, and speech arrest at age of 7. He started using oxcarbazepine. Both patients underwent routine electroencephalography for electroclinical diagnosis and presented a seizure. Although self-limited epilepsy with centrotemporal spikes is a very common epileptic syndrome, seizure visualization is very difficult, and these videos may bring didactical information for recognition of this usual presentation of benign childhood focal epilepsy.

Myoclonic status epilepticus (MSE) in patients without epilepsy, or de novo MSE, is a rare condition associated with several acute symptomatic etiologies, including drugs and toxins. We describe a 94-year-old woman with Alzheimer dementia and long use of mirtazapine 30 mg/d and alprazolam 1 mg/d who developed MSE approximately 24 hours after abrupt discontinuation of alprazolam. The patient was taking sulfamethoxazole/trimethoprim for urinary tract infection, diagnosed 2 weeks before admission. She had no history of seizures. Routine laboratory examinations were normal and head computed tomography showed no acute injuries. She received a loading dose of 1000 mg of intravenous valproate (VPA). Continuous electroencephalogram monitoring revealed very frequent generalized spikes and polyspikes in a markedly slowed background activity. Intravenous VPA 500 mg thrice a day and alprazolam 0.5 mg twice a day were prescribed, and antibiotic was switched to piperacillin/tazobactam. Myoclonic jerks ceased completely and electroencephalogram showed no epileptiform discharges 2 days after VPA treatment onset, with recovery of baseline neurological status. This is, to the best of our knowledge, the first report of de novo MSE related to abrupt discontinuation of benzodiazepines. Seizures and status epilepticus are potential adverse events after abrupt withdrawal of chronically used benzodiazepines, especially in conditions with intrinsic epileptogenic susceptibility, such as Alzheimer disease.

PURPOSE In comatose patients, distinguishing between nonconvulsive status epilepticus and diffuse structural or metabolic encephalopathies is often challenging. Both conditions can generate periodic discharges on EEG with similar morphology and periodicity. We investigated the occurrence of high-frequency oscillations-potential biomarkers of epileptogenesis-on scalp EEG of comatose patients with periodic discharges in the EEG. METHODS Fifteen patients were included. Patients were divided into three groups, according to underlying etiology: Group 1, seizure related; group 2, structural; group 3, nonstructural. EEG recordings were compared with respect to the presence and rates of gamma (30-80 Hz) and ripples (80-250 Hz). RESULTS Patients were 23 to 106 years old (median, 68 years); 60% were female. 206 channels were eligible for analysis (median, 15 channels/patient). Overall, 43% of channels showed gamma, and 24% had ripples. Group 2 showed the highest proportion of channels with gamma (47%), followed by group 1 (38%) and group 3 (36%). Mean gamma rates were higher in group 2 (4.65 gamma/min/channel) than in group 1 (1.52) and group 3 (1.44) (P < 0.001). Group 2 showed the highest proportion of channels with ripples (29.2%), followed by group 1 (15%) and group 3 (24.2%). Mean ripple rates were higher in group 2 (5.09 ripple/min/channel) than in group 1 (0.96) and group 3 (0.83) (P < 0.001). CONCLUSIONS Fast oscillations, including high-frequency oscillations, can be detected in scalp EEG of patients with altered consciousness. High rates of fast activity may suggest an underlying structural brain lesion. Future studies are needed to determine whether fast oscillations in the setting of acute/subacute brain lesions are a biomarker of subsequent development of human epilepsy.

We conducted a retrospective study in order to investigate the clinical significance of temporopolar grey/white matter abnormalities (GWMA) in patients with temporal lobe epilepsy (TLE) and unilateral hippocampal sclerosis (HS) with a long post-surgical follow-up.The study comprised 122 consecutive patients with medically refractory TLE and unilateral HS who underwent epilepsy surgery and had a minimum postoperative follow-up of 5 years. Patients were divided into two groups, based on findings of pre-surgical MRI: group 1 with GWMA and 2 with normal signal and grey/white matter definition in temporal pole. Demographic and clinical data were reviewed and compared between groups.GWMA were found in 52.5% of patients, always ipsilateral to HS. Compared with group 2, group 1 patients had earlier epilepsy onset (mean, 9.3 vs 14.4 years, P=0.001), a higher occurrence of first seizure ≤2 years of age (25.8% vs 10.5%, P=0.036; OR=2.96 [95% CI=1.07-8.19]), and greater prevalence of left HS (76.6% vs 43.1%, P0.001; OR=4.31 [95% CI=1.98-9.38]). No differences were found in gender, presence or type of initial precipitating injury, history of secondary generalized seizures, duration of epilepsy, seizure frequency before surgery, neuropsychological evaluation and presence or lateralization of pre-surgical interictal epileptiform discharges. Postoperative follow-up varied from 5 to 11.5 years (mean 7.4) and was similar in both groups (P=0.155). The proportion of patients classified as seizure-free (Engel class I) at last follow-up in groups 1 and 2 were 73.4% and 69%, respectively (P=0.689). Similarly, the percentages of seizure-free patients with no antiepileptic drugs at last evaluation were not different between groups (P=0.817). In logistic regression analysis, left HS (P=0.001; OR=4.166 [95% CI=1.86-9.34]) and age at epilepsy onset ≤2 years (P=0.047; OR=3.885 [95% CI=1.86-17.50]) were independently associated with risk of having GWMA.GWMA are frequent findings in patients with TLE and HS, and may help lateralize the epileptogenic zone. Our data support the hypothesis that GWMA are caused by seizure-related insults during the critical period of cerebral myelination. GWMA did not influence the postoperative seizure outcome of patients with TLE and HS, even after an extended duration of post-surgical follow-up.

Introduction Nonconvulsive seizures (NCS) are frequent in hospitalized patients and may further aggravate injury in the already damaged brain, potentially worsening outcomes in encephalopathic patients. Therefore, both early seizure recognition and treatment have been advocated to prevent further neurological damage. Objective Evaluate the main EEG patterns seen in patients with impaired consciousness and address the effect of treatment with antiepileptic drugs (AEDs), continuous intravenous anesthetic drugs (IVADs), or the combination of both, on outcomes. Methods This was a single center retrospective cohort study conducted in a private, tertiary care hospital. Consecutive adult patients with altered consciousness submitted to a routine EEG between January 2008 and February 2011 were included in this study. Based on EEG pattern, patients were assigned to one of three groups: Group Interictal Patterns (IP; EEG showing only interictal epileptiform discharges or triphasic waves), Group Rhythmic and Periodic Patterns (RPP; at least one EEG with rhythmic or periodic patterns), and Group Ictal (Ictal; at least one EEG showing ictal pattern). Groups were compared in terms of administered antiepileptic treatment and frequency of unfavorable outcomes (modified Rankin scale ≥3 and in-hospital mortality). Results Two hundred and six patients (475 EEGs) were included in this analysis. Interictal pattern was observed in 35.4% (73/206) of patients, RPP in 53.4% (110/206) and ictal in 11.2% (23/206) of patients. Purpose Gender differences are recognized in the functional and anatomical organization of the human brain. Differences between genders are probably expressed early in life, when differential rates of cerebral maturation occur. Sexual dimorphism has been described in temporal lobe epilepsy with mesial temporal sclerosis (TLE-MTS). Several voxel-based morphometry (VBM) studies have shown that TLE-MTS extends beyond mesial temporal structures, and that there are differences in the extent of anatomical damage between hemispheres, although none have approached gender differences. Our aim was to investigate gender differences and anatomical abnormalities in TLE-MTS. Methods VBM5 was employed to analyze gender and hemispheric differences in 120 patients with TLE-MTS and 50 controls. Results VBM abnormalities were more widespread in left-TLE; while in women changes were mostly seen in temporal areas, frontal regions were more affected in men. Conclusions Our study confirmed that gender and laterality are important factors determining the nature and severity of brain damage in TLE-MTS. Differential rates of maturation between gender and hemispheres may explain the distinct areas of anatomical damage in men and women.

The idea that magnetic fields could be used therapeutically arose 2000 years ago. These therapeutic possibilities were expanded after the discovery of electromagnetic induction by the Englishman Michael Faraday and the American Joseph Henry. In 1896, Arsène d’Arsonval reported his experience with noninvasive brain magnetic stimulation to the scientific French community. In the second half of the 20th century, changing magnetic fields emerged as a noninvasive tool to study the nervous system and to modulate neural function. In 1985, Barker, Jalinous, and Freeston presented transcranial magnetic stimulation, a relatively focal and painless technique. Transcranial magnetic stimulation has been proposed as a clinical neurophysiology tool and as a potential adjuvant treatment for psychiatric and neurologic conditions. This article aims to contextualize the progress of use of magnetic fields in the history of neuroscience and medical sciences, until 1985.

Gastaut type idiopathic childhood occipital epilepsy is an uncommon epileptic syndrome characterised by frequent seizures, most commonly presenting as elementary visual hallucinations or blindness. Other occipital (non-visual) symptoms may also occur. Interictal EEG typically shows occipital paroxysms, often with fixation-off sensitivity. Ictal EEG is usually characterised by interruption by paroxysms and sudden appearance of low-voltage, occipital, fast rhythm and/or spikes. Despite well described clinical and EEG patterns, to our knowledge, there are very few reports in the literature with video-EEG recording of either seizure semiology or fixation-off phenomena. We present a video-EEG recording of a 12-year-old girl with Gastaut type epilepsy, illustrating the interictal and ictal aspects of this syndrome. Our aim was to demonstrate the clinical and neurophysiological pattern of a typical seizure of Gastaut type epilepsy, as well as the fixation-off phenomena, in order to further clarify the typical presentation of this syndrome. This case report describes neurotoxicity in an older patient on cefepime 2 g twice a day. The 81-year-old male patient developed non-convulsive status epilepticus during cefepime treatment with 1 g twice a day. There was recovery 30 days after discontinuation of cefepime.

Summary Psychiatric outcomes of patients submitted to epilepsy surgery have gained particular interest given the high prevalence of pre-surgical psychiatric disorders (PD) in this population. The present study aimed to verify the possible pre-surgical predictors for psychiatric disorders following epilepsy surgery in a homogeneous series of patients with refractory temporal lobe epilepsy and mesial temporal sclerosis (TLE-MTS). Data from 115 TLE-MTS patients (65 females; 56.5%) who underwent cortico-amygdalohippocampectomy were included. Pre- and post-surgical psychiatric evaluations were performed using DSM-IV criteria. Pre-surgical PD – particularly mood, anxiety and psychotic disorders – were diagnosed in 47 patients (40.8%). Twenty-seven patients (54% of those with pre-surgical PD) demonstrated a remission of psychiatric symptoms on post-surgical psychiatric evaluation. Eleven patients (9.6%) developed de novo PD. The presence of pre-surgical depression (OR=3.32; p =0.008), pre-surgical interictal psychosis (OR=4.39; p =0.009) and epileptiform discharges contralateral to the epileptogenic zone (OR=2.73; p =0.01) were risk factors associated with post-surgical PD. Although epilepsy surgery is considered to be the best treatment option for patients with refractory TLE-MTS, the relatively high psychiatric comorbidities observed in surgical candidates and their possible negative impact on post-surgical outcomes require a careful pre-surgical evaluation of clinical, sociodemographic and psychiatric factors.

Purpose The association between pre-surgical psychiatric disorders (PDs) and worse seizure outcome in patients with refractory epilepsy submitted to surgery has been increasingly recognized in the literature. The present study aimed to verify the impact of pre- and post-surgical PD on seizure outcome in a series of patients with refractory temporal lobe epilepsy and mesial temporal sclerosis (TLE–MTS). Method Data from 115 TLE–MTS patients (65 females; 56.5%) who underwent cortico-amygdalohippocampectomy (CAH) were analyzed. Pre- and post-surgical psychiatric evaluations were performed using DSM-IV and ILAE criteria. The outcome subcategory Engel IA was considered as corresponding to a favorable prognosis. A multivariate logistic regression model was applied to identify possible risk factors associated with a worse seizure outcome. Results Pre-surgical PDs, particularly major depressive disorder (MDD), anxiety and psychotic disorders, were common, being found in 47 patients (40.8%). Fifty-six patients (48.7%) were classified as having achieved an Engel IA one year after CAH. According to the logistic regression model, the presence of pre-surgical MDD (OR=5.23; p =0.003) appeared as the most important risk factor associated with a non-favorable seizure outcome. Conclusion Although epilepsy surgery may be the best treatment option for patients with refractory TLE–MTS, our findings emphasize the importance of performing a detailed psychiatric examination as part of the pre-surgical evaluation protocol.

The advent of modern image-guided surgery has revolutionized depth electrode implantation techniques. Stereoelectroencephalography (SEEG), introduced by Talairach in the 1950s, is an invasive method for three-dimensional analysis on the epileptogenic zone based on the technique of intracranial implantation of depth electrodes. The aim of this article is to discuss the principles of SEEG and their evolution from the Talairach era to the image-guided surgery of today, along with future prospects. Although the general principles of SEEG have remained intact over the years, the implantation of depth electrodes, i.e. the surgical technique that enables this method, has undergone tremendous evolution over the last three decades, due the advent of modern imaging techniques, computer systems and new stereotactic techniques. The use of robotic systems, the constant evolution of imaging and computing techniques and the use of depth electrodes together with microdialysis probes will open up enormous prospects for applying depth electrodes and SEEG both for investigative use and for therapeutic use. Brain stimulation of deep targets and the construction of "smart" electrodes may, in the near future, increase the need to use this method.

Machado de Assis (1839–1908)—novelist, short story writer, essayist, and poet—is a fascinating personality. Had he written in French, English, German, or Italian, he would have achieved universal fame and would be in the same company as Balzac, Tolstoy, Dickens, and Dostoevsky. This article discusses stigma in epilepsy through Machado de Assis’ life, literary work, and letters to other Brazilian writers. Founder of the Brazilian Academy of Letters, Machado offers an insoluble enigma to psychologists and essayists. Born in stark poverty, feeble, and ugly, he had to fight the taint of epilepsy. The documentation of epilepsy in Machado de Assis’ texts and letters and the testimony of his contemporaries is unique, allowing the comprehension of scientific concepts and stigma related to epilepsy in the 19th century, when the positivist ideas of the Italian neuropsychiatrist Cesare Lombroso (1835–1909) permeated nascent Brazilian neuropsychiatry. Much of the stigma associated with epilepsy we witness today emerged from these concepts. Even today in Brazil, when barbaric crimes are committed, headlines in newspapers produced by forensic psychiatrists often attribute the conduct of the criminal to epileptic behavior.

Introduction Prognostication after cardiac arrest (CA) poses a challenge to physicians, as it may lead to futile treatment in some cases, and withdrawal of life support therapy (WLST) in others. Clinical and neurophysiological data – electroencephalogram (EEG) and somatosensory evoked potential (SSEP) – are used as prognostic features. In this study, we aimed at analyzing data of patients who survived CA, in a hospital setting in Brazil, where care is not withdrawn even in patients with expected poor neurological outcome. Methods We reviewed clinical and neurophysiological data of patients who had EEG after CA from 2006 to 2016. We considered background suppression, burst-suppression, periodic discharges, and status epilepticus (SE) as ‘malignant’ EEG patterns. Neurologic disability was measured according to the modified Rankin scale (mRS). Scores from 4 to 6 were considered poor outcome. Results 32 patients had EEG after CA. Age ranged from 47 to 98 years (mean 71.6). The majority of patients had in-hospital CA (75%) and the most common cause was hypoxia. 37% received therapeutic hypothermia (TH) as standard care. 73% of patients had poor outcome; 63% of these had a ‘malignant’ EEG pattern after CA. However, 45% of patients who had favorable outcome also had a ‘malignant’ EEG pattern after CA. Patients who received TH presented more ‘malignant’ EEG patterns, compared with those who did not (66% vs 50%). ‘Malignant’ EEG patterns were seen in most of the patients, regardless of CA local (in-hospital 54% vs out-of-hospital 62%). Conclusion Our results emphasize that good outcome may be achieved even if face of ‘malignant’ EEG patterns, and this should be considered when WLST is evaluated.

Objective The aim of the study described here was to examine the relationship between memory function, proton magnetic resonance spectroscopy (1H-MRS) abnormalities, and interictal epileptiform discharge (IED) lateralization in patients with temporal lobe epilepsy (TLE) related to unilateral mesial temporal sclerosis. Methods We assessed performance on tests of memory function and intelligence quotient (IQ) in 29 right-handed outpatients and 24 controls. IEDs were assessed on 30-minute-awake and 30-minute-sleep EEG samples. Patients had 1H-MRS at 1.5 T. Results There was a negative correlation between IQ (P = 0.031) and Rey Auditory Verbal Learning Test results (P = 0.022) and epilepsy duration; between1H-MRS findings and epilepsy duration (P = 0.027); and between N-acetylaspartate (NAA) levels and IEDs (P = 0.006) in contralateral mesial temporal structures in the left MTS group. 1H-MRS findings, IEDs, and verbal function were correlated. Conclusions These findings suggest that IEDs and NAA/(Cho + Cr) ratios reflecting neural metabolism are closely related to verbal memory function in mesial temporal sclerosis. Higher interictal activity on the EEG was associated with a decline in total NAA in contralateral mesial temporal structures.

OBJETIVO: Avaliar o estado empregatício e a qualidade de vida (QV) de indivíduos com epilepsia do lobo temporal mesial (ELTM) com esclerose do hipocampo (EH) antes e três anos após a realização de corticoamigdalohipocampectomia e verificar as variáveis demográficas e clínicas associadas ao emprego. METODOLOGIA: 58 pacientes com ELTM com EH submetidos a corticoamigdalohipocampectomia foram avaliados antes e três anos após a cirurgia. Todos responderam ao Epilepsy Surgery Inventory (ES-I55) - versão brasileira como medida da QV, bem como a um breve questionário contendo dados sociodemográficos e clínicos. Alterações na situação de emprego após a cirurgia foram classificadas do seguinte modo: (i) melhora: indivíduos desempregados, com emprego não-formal, estudantes, donas-de-casa e aqueles que nunca haviam trabalhado e que estavam empregados três anos após a cirurgia; (ii) nenhuma mudança: aqueles que não obtiveram modificações em sua situação ocupacional. Esta categoria compreendeu indivíduos que permaneceram empregados, continuaram com atividades domésticas, estudantes, em auxílio doença, aposentados e os que nunca trabalharam; e (iii) piora: perda do emprego. RESULTADOS: A situação de emprego não mudou significativamente após a cirurgia: 51(87.9%) permaneceram com o mesmo estado empregatício anterior à cirurgia, seis (10.3%) tiveram melhora, e um paciente (1.7%), que estava empregado antes da cirurgia, aposentou-se. No subgrupo dos 22 pacientes empregados após três anos, dez (45.5%) estavam livres de crises, sete (31.8%) tinham apenas auras esporádicas e cinco (22.7%) permaneceram com crises. No grupo que obteve melhora, 12 pacientes (70.5%) eram autônomos antes da cirurgia e cinco (29.5%), tinham emprego formal. Na avaliação do terceiro ano após cirurgia, 14 (63.6%) dos 22 indivíduos conseguiram um emprego formal. Objective The purpose of this article was to report the translation of the Quality of Life in Epilepsy Inventory–89 (QOLIE-89) into a Portuguese–Brazilian version and evaluate its reliability and validity. Methods This study involved 105 outpatients: 54 patients with refractory temporal lobe epilepsy (TLE) with mesial temporal sclerosis (MTS) and 51 with juvenile myoclonic epilepsy (JME). Reliability and test–retest reliability were assessed. Relationships between QOLIE-89 domains and other questionnaires (Nottingham Health Profile, Beck Depression Inventory, Adverse Event Profile, Neuropsychological Evaluation), and external measures such as demographic and clinical variables were analyzed to examine construct validity. Results Internal consistency (Cronbach’s α = 0.73–0.92) and test–retest reliability (intraclass correlation coefficient = 0.60–0.84) for individual domains were acceptable. For construct validity, we verified high correlations between the QOLIE-89 and the Nottingham Health Profile, Beck Depression Inventory, Adverse Event Profile, and Neuropsychological Evaluation. For clinical characteristics, the patients with juvenile myoclonic epilepsy had better quality-of-life scores on 11 of 17 QOLIE-89 subscales compared with patients with temporal lobe epilepsy ( P Conclusion These results support the reliability and validity of the Portuguese–Brazilian translation of QOLIE-89.

The objective of the study was to describe the clinical characteristics and determine the lateralizing value of ictal Sign of the Cross (SC) as a complex hand automatism (CHA) in patients evaluated by video/EEG monitoring in a comprehensive epilepsy unit. We reviewed video/EEG data of 530 patients with epilepsy recorded in a tertiary epilepsy center from 2002 to 2008. Four patients were found to have manifested a CHA similar to the SC at least once during their complex partial seizures. All patients had unilateral right mesial temporal lobe epilepsy (TLE) refractory to medical treatment. The limbic system is often suggested as the critical site of religious experience. Moreover, it may be localized predominantly to the temporal regions of the right hemisphere. However, this rare and peculiar ictal manifestation may be related not only to the neural substrate and personality characteristics of TLE, but also to the general religious convictions of Brazilians.

To investigate whether interdigestive gastric acid secretion can be controlled by a possible memory-related cortical mechanism.To evaluate gastric secretion in rats, we used a methodology that allows gastric juice collection in rats in their habitual conditions (without any restraining) by pairing sound as the conditioning stimulus (CS) and food as the unconditioning stimulus (US). The levels of gastric acid secretion under basal conditions and under sound stimulation were recorded and the circulating gastrin levels determined.When the gastric juice was collected in the course of the conditioning procedure, the results showed that under noise stimulation a significant increase in gastric acid secretion occurred after 10 days of conditioning (p0.01). The significance was definitively demonstrated after 13 days of conditioning (p0.001). Basal secretions of the conditioned rats reached a significant level after 16 days of conditioning. The levels of noise-stimulated gastric acid secretion were the highest so far described in physiological experiments carried out in rats and there were no significant increases in the circulating gastrin levels.The results point to the important role played by cortical structures in the control of interdigestive gastric acid secretion in rats. If this mechanism is also present in humans, it may be involved in diseases caused by inappropriate gastric acid secretion during the interprandial periods.

INTRODUÇÃO: Embora a fotostimulação intermitente (FEI) seja rotineiramente utilizada nos laboratórios de eletrencefalografia (EEG), raramente é utilizada de forma padronizada. A FEI é um método de ativação utilizado no EEG de rotina que pode desencadear tanto respostas fisiológicas quanto potencialmente patológicas. Historicamente, o termo fotossensibilidade se refere às respostas anormais à estimulação com luz estroboscópica durante o registro do EEG. OBJETIVO: O objetivo desta publicação é revisar os aspectos diagnósticos do procedimento da FEI, baseados no encontro de consenso realizado em Heemstede na Holanda, em 1996, com o propósito de facilitar e padronizar a detecção de pacientes fotossensíveis. INTRODUCTION: Although intermittent photic stimulation (IPS) is a widespread and routinely used procedure in EEG laboratories, only relatively recently has a standardization of the IPS method been proposed. IPS is an activation method used during EEG procedure that may trigger either physiological or potentially pathological responses. Historically, the term photosensitivity is referred to abnormal responses to stroboscopic light during EEG. OBJECTIVE: The purpose of this publication was to provide an overview of the diagnostic aspects of IPS procedure, based on data presented at Consensus Meetings held in Heemstede, the Netherlands, in 1996, with the purpose of facilitating the detection of photosensitive patients.

OBJETIVO: Este estudo teve como objetivo avaliar a qualidade de vida (QV) em três diferentes grupos de epilepsia e verificar a esfera percebida como de maior impacto na vida diária. METODOLOGIA: A amostra foi composta por 57 pacientes com epilepsias focais e generalizadas do Hospital São Paulo, Universidade Federal de São Paulo (UNIFESP), divididos em três grupos, epilepsias do lobo temporal (ELT), extratemporais (Extra-ELT) e generalizadas idiopáticas (EGI). Os pacientes responderam a um questionário preliminar para identificar a percepção sobre os aspectos mais comprometidos em suas vidas. A QV foi avaliada por meio da versão brasileira do Quality of Life in Epilepsy Inventory 31 (QOLIE-31). A correlação dos domínios do QOLIE-31 com a duração da epilepsia e freqüência de crises foi definida pela inspeção dos gráficos de dispersão e pela correlação de Pearson e de Spearman. Foram considerados significantes os valores de p < 0,05. RESULTADOS: Dificuldades nas relações interpessoais, familiares e sociais foram apontadas como a esfera de maior impacto relacionado à epilepsia, citada por 13 (22,81%) pacientes. O QOLIE-31 mostrou resultado semelhante nos três diferentes tipos de epilepsia, com exceção do domínio Funcionamento Cognitivo. Os escores deste domínio foram significativamente menores (p = 0,01) no grupo com Extra-ELT (38,4) do que nos grupos ELT (52,6) e EGI (62,6). A duração da epilepsia não influenciou na QOL nesta amostra, porém foi observada uma correlação estatística significante entre a freqüência de crises e os domínios Efeitos da Medicação (p = 0,0476, alfa = 0,05) e Preocupação com Crises (p = 0,0463, alfa = 0,05). A freqüência de crises mostrou ainda uma diferença estatisticamente significante (p = 0.007) no grupo com Extra-ELT, que apresentou mais crises/paciente, quando comparado aos demais grupos. CONCLUSÕES: Os pacientes identificaram as relações interpessoais, familiares e sociais como sendo a área mais afetada pela epilepsia. O domínio Funcionamento Cognitivo mostrou-se como fator determinante na QOL do grupo Extra-ELT. A freqüência de crises influenciou a QOL nos domínios Preocupação com Crises e Efeitos da Medicação nos três grupos. PURPOSE: This study aimed to evaluate the quality of life (QOL) and verify the domains of greater impact in patients with focal and generalized epilepsies. METHODS: The sample, composed by 57 subjects from Hospital São Paulo da Universidade Federal de São Paulo (UNIFESP), was divided into 3 groups, temporal lobe epilepsies (TLE), extra-temporal epilepsies (Extra-TLE) and idiopathic generalized epilepsy (IGE). They answered a preliminary self-reported questionnaire to identify the perception of the most impaired aspects in their lives. The QOL was evaluated through the validated Brazilian version of the Quality of Life Epilepsy Inventory 31 (QOLIE-31). The correlation of the QOLIE-31 domains with epilepsy duration and seizure frequency was defined by dispersion graphics and also Pearson s and Spearman s correlation. RESULTS: The most frequently identified impact of epilepsy was related to interpersonal, familial and social relationships mentioned by 13 (22.81%) patients. The seizure frequency per patient in Extra-TLE group was significantly greater (p = 0.007) than in the other groups. The Cognition Functioning scores were lower for the Extra-TLE group (38.4) when compared with TLE (52.6) and IGE (62.6) (p = 0.01). The correlation between epilepsy duration and QOLIE-31 domains did not demonstrate statistical significance; however, seizure frequency was correlated with Seizures Worry (p = 0.0463, alpha = 0.05) and Medication Effects (p = 0.0476, alpha = 0.05) domains. Summary: Purpose: To assess the frequency and significance of facial paresis (FP) in a well-defined cohort of mesial temporal lobe epilepsy (MTLE) patients. Methods: One hundred consecutive patients with MRI findings consistent with mesial temporal sclerosis (MTS) and concordant electroclinical data underwent facial motor examination at rest, with voluntary expression, and with spontaneous smiling. Hippocampal, amygdaloid, and temporopolar (TP) volumetric measures were acquired. Thirty healthy subjects, matched according to age and sex, were taken as controls. Results: Central-type FP was found in 46 patients. In 41 (89%) of 46, it was visualized at rest, with voluntary and emotional expression characterizing true facial motor paresis. In 33 (72%) of 46 patients, FP was contralateral to the side of MTS. By using a 2-SD cutoff from the mean of normal controls, we found reduction in TP volume ipsilateral to MTS in 61% of patients with FP and in 33% of those without (p = 0.01). Febrile seizures as initial precipitating injury (IPI) were observed in 34% of the patients and were classified as complex in 12 (26%) of 46 of those with FP and in five (9%) of 54 of those without (p = 0.02). The presence of FP was significantly associated with a shorter latent period and younger age at onset of habitual seizures, in particular, with secondarily generalized tonic‐clonic seizures. Conclusions: Facial paresis is a reliable lateralizing sign in MTLE and was associated with history of complex febrile seizures as IPI, younger age at onset of disease, and atrophy of temporal pole ipsilateral to MTS, indicating more widespread disease. Purpose We report the cultural adaptation and psychometric properties of the Quality of Life in Epilepsy-31 Inventory (QOLIE-31) for the Portuguese language and Brazilian culture. Methods This study involved 150 outpatients: 50 presurgical patients with refractory temporal lobe epilepsy (TLE) related to mesial temporal sclerosis (MTS), 50 patients with juvenile myoclonic epilepsy (JME), and 50 seizure-free patients with TLE. They completed the QOLIE-31, Nottingham Health Profile (NHP), Beck Depression Inventory (BDI), and Adverse Events Profile (AEP) and underwent a neuropsychological evaluation (NE). Internal consistency reliability, interrater and test–retest reliability, and construct validity were assessed. Results QOLIE-31 mean scores were 33.1 (Social Function), 68.9 (Overall Quality of Life), 56.5 (Seizure Worry), 64.1 (Emotional Well-Being), 63.7 (Energy/Fatigue), 38.9 (Cognitive Function), and 49.7 (Medication Effects). Internal consistency was high (Cronbach’s α), as were the associations between QOLIE-31 and the BDI, NHP, AEP, and NE. Conclusion The Portuguese/Brazilian version of the QOLIE-31 inventory showed good reliability, validity, and construct validity.

Objective: the objective of this retrospective study is to analyze ictal patterns observed during continuous Video-EEG monitoring in patients with temporal Lobe epilepsy (TLE) due to unilateral hippocampal sclerosis (HS), and to correlate these EEG patterns to temporal pole abnormalities observed on magnetic resonance imaging exams.Methods: We analyzed 147 seizures from 35 patients with TLE and unilateral HS. Ictal patterns were classified.,and correlated to signal abnormalities and volumetric measures of the temporal poles. Volume differences over 10% were considered abnormal.Results: the most frequent type of ictal pattern was rhythmic theta activity (RTA), encountered in 65.5% of the seizures. Rhythmic beta activity (RBA) was observed in 11% of the seizures, localized attenuation in 8%, interruption of epileptiform discharges in 6%, repetitive discharges in 5.5%, and rhythmic delta activity (RDA) in 4%. Sixty-six percent of the patients presented signal abnormalities in the temporal pole that were always ipsitateral to the HS. Sixty percent presented significant asymmetry of the temporal poles consisting of reduced volume that was also always ipsitateral to HS. Although patients with RTA as the predominant ictal pattern tended to present asymmetry of temporal poles (p = 0.305), the ictal EEG pattern did not correlate with temporal pole asymmetry or signal abnormalities.Conclusions: RTA is the most frequent initial ictal pattern inpatients with TLE due to unilateral HS. Temporal pole signal changes and volumetric reduction were commonly found in this group of patients, both abnormalities appearing always ipsitateral to the HS. However, neither temporal pole volume reduction nor signal abnormalities correlated with the predominant ictal pattern, suggesting that the temporal poles are not crucially involved in the process of epileptogenesis. INTRODUCTION: Surgery is widely accepted as a modality of treatment for medically intractable seizures and the main goals of surgery as reduction or elimination of seizures without significant compromise in neurological function and improving QOL. PURPOSE: To assess QOL pre- and post-surgery in a sample of patients with temporal lobe epilepsy (TLE) due to mesial temporal sclerosis (MTS). METHODS: Thirty-five consenting subjects who had undergone corticoamygdalo-hipocampetomy were included in this study. They answered the ESI-55 before and six months after surgical treatment. Analyses of Variance (ANOVA) was used to study the changes in QOL and we also determined the effect-size of the sample. RESULTS: After surgical treatment 18 (51%) subjects were seizure free, 10 (29%) had only auras and seven (20%) had partial seizures. The post-operative mean scores were higher than pre-operative scores, except for Cognitive Function and Limitation due to Cognitive Problems in the group with seizures. A significant improvement in QOL post-surgery was observed in the domains Health-Perception (1.24), Emotional-Well-being (1.32) and Energy/Fatigue (1.43). CONCLUSION: A long-term follow-up is necessary to identify meanigful changes after the surgery.

INTRODUCTION: Somatosensory auras (SSAs) are more usually described in patients with parietal lobe epilepsy (PLE), producing more commonly a localized cutaneous tingling sensation, involving hands and fingers followed by tonic or clonic focal seizures. These usually originate in the contralateral hemisphere. Etiology includes dysplasias, tumours, ischemic or postencephalitic gliosis. However, other focal epilepsies, such as frontal and temporal, may also originate SSAs. Although this type of aura is reported as rare in patients with mesial temporal lobe epilepsy (MTLE), this association has not been systematically studied. OBJECTIVES: The aim of this article was to describe the cases of four patients with refractory MTLE and SSAs, reporting their clinical characteristics and MRI findings. We discuss the localizing and lateralizing value of SSAs, particularly in the context of MTLE. METHODS AND RESULTS: Four patients with refractory MTLE and SSAs followed-up in the outpatient's clinic at the Epilepsy Section, Universidade Federal de São Paulo, were submitted to presurgical evaluation and corticoamygdalohippocampectomy. MRI in all cases showed unilateral mesial temporal sclerosis (MTS). Regarding seizure semiology, tingling sensation involving the upper extremity was the most prevalent symptom. Three of the four patients had SSAs contralateral to the MTS. Following the SSAs all patients most of the time presented other symptoms such as autonomic or psychic auras evolving to psychomotor seizures. After surgical treatment, two of the patients presented infrequent auras, and two were rendered seizure-free. CONCLUSION: Although rare, SSAs can be present in MTLE. The characteristics of autonomic or psychic auras, psychomotor seizures, neuropsychological deficits, and typical neurophysiologic and MRI findings may help differentiate patients with MTLE from those with PLE.

Summary Purpose Ictal spitting is rarely reported in patients with epilepsy. More often it is observed in patients with temporal lobe epilepsy (TLE) and is presumed to be a lateralizing sign to language nondominant hemisphere. We report three patients with left TLE who had ictal spitting registered during prolonged video-EEG monitoring. Methods Medical charts of all patients with medically refractory partial epilepsy submitted to prolonged video-EEG monitoring in the Epilepsy Unit at UNIFESP during a 3-year period were reviewed, in search of reports of ictal spitting. The clinical, neurophysiological and neuroimaging data of the identified patients were reviewed. Results Among 136 patients evaluated with prolonged video-EEG monitoring, three (2.2%) presented spitting automatisms during complex partial seizures. All of them were right-handed, and had clear signs of left hippocampal sclerosis on MRI. In two patients, in all seizures in which ictal spitting was observed, EEG seizure onset was seen in the left temporal lobe. In the third patient, ictal onset with scalp electrodes was observed in the right temporal lobe, but semi-invasive monitoring with foramen ovale electrodes revealed ictal onset in the left temporal lobe, confirming false lateralization in surface records. The three patients became seizure-free following left anterior temporal lobectomy. Conclusions Ictal spitting is a rare finding in patients with epilepsy, and may be considered a localizing sign of seizure onset in the temporal lobe. It may be observed in seizures originating from the left temporal lobe, and thus should not be considered a lateralizing sign of nondominant TLE.

The purpose of this study was to develop a Portuguese version of the Epilepsy Surgery Inventory (ESI-55) and to assess its psychometric properties. Sixty patients with temporal lobe epilepsy related to unilateral mesial temporal sclerosis who underwent presurgical evaluation at the Universidade Federal de São Paulo (UNIFESP) formed the sample for this study. The psychometric properties of the ESI-55 included: reliability, validity, and responsiveness. Internal consistency was high in all domains (Cronbach's alpha ranging from 0.76 for Social Function to 0.88 for Physical Function) except Overall Quality of Life (alpha=0.45). Test-retest reliability after 1 week was good, with the intraclass correlation coefficient ranging from 0.79 (Energy/Fatigue) to 0.92 (Role Limitations due to Emotional Problems). Interrater reliability ranged from 0.84 (Cognitive Function) to 0.94 (Role Limitations due to Physical Problems). For construct validity, we verified a high correlation between the ESI-55 and Health Assessment Questionnaire-8 for the Physical Function domain (Pearson linear correlation=-0.84), and a moderate correlation for the Pain domain (P=-0.58), but for the other subscales no correlation was detected. Beck Depression Inventory and ESI-55 domains were highly statistically correlated (ANOVA: P0.005), but there was no association of the Cognitive Function and Role Limitations due to Memory Problems subscales with neuropsychological evaluation (Pearson coefficient: P0.05). With respect to demographic characteristics, a statistically significant correlation was observed for the variable educational level (Student t, P0.005) and ESI-55 scores. There was a high correlation between seizure frequency and ESI-55 domains for clinical variables (ANOVA, P0.005). Surgical treatment in this series improved health-related quality of life in the seizure-free group in three domains--Health Perception (1.24), Emotional Well-Being (1.32), and Energy/Fatigue (1.48)-as reflected by the standard response mean and the effect size of the sample. Our results support the psychometric properties of the Portuguese version of the ESI-55 as a measure of health-related quality of life.

INTRODUÇÃO: A obtenção de informações sobre o funcionamento e bem-estar dos pacientes é essencial para avaliação dos benefícios das intervenções terapêuticas, porque fornece evidências sobre o impacto da doença e do tratamento em termos de condição de saúde. No Brasil não dispomos de literatura sobre tradução, adaptação cultural e validação de escalas de qualidade de vida (QV) desenvolvidas para avaliar pessoas com epilepsia. OBJETIVO: Tradução e adaptação cultural do Questionário Qualidade de Vida em Epilepsia-31 [Quality of Life in Epilepsy Inventory-31 (QOLIE-31)]. CASUÍSTICA E METODOLOGIA: Trinta pacientes em acompanhamento regular com diagnóstico de epilepsia do lobo temporal por esclerose mesial temporal (n = 18) e epilepsia mioclônica juvenil (n = 12) responderam ao questionário. A versão original do questionário foi obtida com um dos autores da escala (Joyce Cramer) que concedeu a versão original em inglês para a tradução. RESULTADOS: Dois professores de inglês nativos realizaram a retrotradução. As versões em português e a retrotraduzida foram comparadas à original e após consenso foi obtida a versão final. Vinte pacientes (66%) eram do sexo feminino e 10 (34%) masculino, com média de idade de 36 anos. Dez (30%) completaram o 1º, 16 (53%) o 2º e 4 (17%) o 3º graus. Dos 30, 14 (46%) apresentaram até 5 crises no último mês, 10 (33%) de 6 a 10 e 6 (21%) mais de 10. Após aplicação dos 30 questionários apenas a questão 28 não foi compreendida por 3 (10%) dos pacientes, sendo reescrita após consultar a autora do questionário. CONCLUSÃO: O QOLIE-31 demonstrou-se uma escala de fácil aplicação na população brasileira, com fácil entendimento. Acreditamos que ao terminar o processo de validação será um instrumento de grande utilidade para avaliar a QV de pessoas com epilepsia.

OBJETIVOS: Comparar os resultados em uma prova cognitiva de pacientes com epilepsia com dois parâmetros: um fornecido pelas normas brasileiras e outro pelas norte-americanas, tendo em vista as diferenças culturais. MÉTODO: Quarenta e cinco pacientes adultos com diagnóstico de epilepsia foram submetidos à avaliação de nível intelectual por meio da Escala Wechsler de Inteligência, WAIS-III nos seguintes itens: Quociente de Inteligência Verbal (QI V), Quociente de Inteligência de Execução (QI E) e Quociente de Inteligência Total (QI T). As correções dos protocolos seguiram as normas brasileiras e as norte-americanas. RESULTADOS: Dos 45 pacientes, 30 tinham o diagnóstico de epilepsia do lobo temporal (ELT) por esclerose mesial temporal (EMT), 15 no hemisfério esquerdo e 15 no direito, e 15 de epilepsia mioclônica juvenil (EMJ). Em todos os 45 protocolos do WAIS-III os resultados de QI V, QI E e QI T foram maiores quando empregados os parâmetros brasileiros. Assim, 18 pacientes que alcançaram QI T nas faixas "Média" e "Média Inferior", segundo as normas brasileiras, obtiveram níveis inferiores quando corrigidos pelas normas norte-americanas, ou seja, o QI T destes mesmos pacientes caiu para as faixas "Limítrofe" e "Muito Rebaixado" respectivamente. Nos pacientes com ELT, as diferenças de pontos entre os QI V e QI E não foram concordantes quando cotejados por normas diferentes e em cinco deles o desempenho verbal foi maior do que o motor quando corrigidos pelas normas norte-americanas. Nos pacientes com EMJ, os valores do QI V e QI E foram maiores quando empregadas normas brasileiras. CONCLUSÃO: Os resultados da avaliação cognitiva pelo WAIS-III de pacientes com epilepsia mostram que a interpretação do nível intelectual é muito distinta quando corrigida segundo normas brasileiras ou norte-americanas. Há também diferença na análise qualitativa do desempenho intelectual, uma vez que o estudo das discrepâncias internas variou conforme a norma populacional utilizada. OBJECTIVES: To compare the results of a cognitive task of patients with epilepsy with two parameters: one provided by the Brazilian and the other by the US normative scores viewing the cultural differences. METHOD: Forty-five adult patients diagnosed with epilepsy were submitted to intellectual evaluation through the Wechsler Adult Intelligence Scale, WAIS-III, in the following items: Verbal Intelligence Quotient (V IQ), Performance Intelligence Quotient (P IQ) and Full Scale (FS IQ). The correction of the protocols followed the Brazilian and the US normative scores. RESULTS: Thirty out of the 45 patients were diagnosed with temporal lobe epilepsy (TLE) due to mesial temporal sclerosis (MTS), 15 in the left hemisphere and 15 in the right, and 15 with juvenile myoclonic epilepsy (JME). In all 45 WAIS protocols the results of the V IQ, P IQ and FS IQ were higher when the Brazilian parameters were employed. Therefore, 18 patients that achieved FS IQ in the "Average" and "Low Average" ranges, according to the Brazilian normative scores, got lower levels when corrected by the US normative scores, that is, the FS IQ of these 18 patients fell to the ‘Borderline’ and ‘Extremely Low’ ranges respectively. In the patients with TLE, the differences in the scores between the V IQ and P IQ did not correspond when evaluated by different normative scores and in five of them the verbal was higher than the performance when corrected by the US normative scores. In the patients with JME, the values of V IQ and P IQ were higher when the Brazilian normative scores were employed. CONCLUSION: The results of the cognitive evaluation through the WAIS of patients with epilepsy showed that interpretation of the intellectual level is very dissimilar when corrected by the Brazilian or US normative scores. There is also a difference in the qualitative analysis of the intellectual performance since the study of the inner discrepancy varied according the population normative scores employed.

INTRODUCTION: Seizure severity and seizure frequency reduction are the goals in the treatment of epilepsy. Up to the present, there are no validated instruments or studies emphasizing initial reliability and validity of questionnaires to measure seizure severity into Brazilian Portuguese. PURPOSE: This report describes the translation and cross-cultural adaptation of the Seizure Severity Questionnaire (SSQ), an instrument to evaluate seizure frequency and severity. CASUISTIC AND METHODS: The author conceded the original English version to Portuguese translation. Later, two independent native English-speaking teachers fluent in Portuguese translated this consensus version back into English. Comparison of the back-translation with the original English version showed only a few discrepancies, and the English and Portuguese versions were considered conceptually equivalent. Thirty patients regularly treated with temporal lobe epilepsy related to mesial temporal sclerosis answered the questionnaire. RESULTS: Twenty-two adult patients (73%) were male and mean age 37. Ten (33%) reported only auras and 18 Movements or attitudes during the seizures. Two presented Loss of consciousness. For 13 (43%) there was a long time to recuperate after the event. 12 reported Emotional effects and all patients had Body effects. The majority of patients, 28 (94%) considered their seizures extremely severe and for 23 (77%) the recuperation period was the most bother symptom. The association of seizure frequency and Nottingham Health Profile showed statistical significance for the domains: Emotional well-being (p = 0.046), Pain (p = 0.015) and Sleep (p = 0.003). CONCLUSION: This study explored the cultural adaptation of SSQ and its first results. We also assessed the correlation between seizure frequency and quality of life impact. The instrument SSQ could help to understand the seizure concern in the view of the patient.

INTRODUÇÃO: O padrão de relacionamento que se estabelece entre paciente e cuidador pode comprometer a qualidade de vida e o crescimento de ambos. OBJETIVO: Comparar a qualidade de vida e a sobrecarga dos cuidadores de pacientes com epilepsia do lobo temporal (ELT) com esclerose mesial temporal (EMT) e epilepsia mioclônica juvenil (EMJ). CASUÍSTICA E METODOLOGIA: Foram avaliados 20 cuidadores de pacientes com ELT e 20 cuidadores de pacientes com EMJ, com o Inventário de Qualidade de Vida SF-36 e Questionário de Sobrecarga do Cuidador ("Burden Interview" - Zarit). RESULTADOS: Houve sobrecarga leve a moderada nos dois grupos (30 no de ELT e 21 pontos para o grupo de EMJ), porém não foi observada associação estatisticamente significante entre os grupos. Ao buscarmos uma correlação entre os domínios do SF-36 e a sobrecarga do cuidador para os dois grupos estudados, observou-se apenas no grupo EMJ: estado geral de saúde (p = 0,011), limitações por aspectos emocionais (p = 0,037) e saúde mental (p = 0,002). CONCLUSÃO: Cuidadores de pacientes dos dois grupos são sobrecarregados de forma semelhante. Esta sobrecarga não interferiu significativamente na qualidade de vida dos mesmos na maioria dos domínios embora cuidadores de pacientes com EMJ tenham revelado impacto em domínios que medem aspectos da saúde emocional e física. INTRODUCTION: The relationship pattern between patient and caregiver can influence in quality of life (QOL) and growth of both. OBJECTIVE: To compare QOL and burden over the caregivers of patients with refractory temporal lobe epilepsy (TLE) due to mesial temporal sclerosis and juvenile myoclonic epilepsy (JME). CASUISTIC AND METHODS: 20 caregivers of patients with TLE and 20 of patients with JME were evaluated. The QOL Inventory SF-36 and the Caregiver Burden Questionnaire "Burden Interview" - Zarit - were applied. RESULTS: When QOL of both groups was compared, no relation statistically significant was observed between caregivers for JME and TLE, as well as burden on them. There was a correlation for burden on the caregivers of JME patients and the following domains of SF-36: General Health Status (p = 0.011), Emotional Limitation Aspects (p = 0.037) e Mental Health (p = 0.002). CONCLUSION: The burden on the caregivers of both groups was similar and did not interfere in QOL in most of SF-36 domains. However, caregivers of patients with JME suffered greater impact in domains related to physical and emotional health. In spite of more adequate seizure control in JME patients this burden might be heavier due to greater exposure to daily activities.

Introdução: Dificuldades no trabalho, em relacionamentos interpessoais, familiares e sociais, a percepção do estigma, da discriminação dentre outros têm sido associados aos estados depressivos interictais, influenciando negativamente a qualidade de vida (QV) das pessoas com epilepsia. A depressão tem alta prevalência (20 a 55%) nas epilepsias, sendo vista como o fator mais importante no julgamento do paciente sobre sua QV. Objetivos: Avaliar a ocorrência de sintomas depressivos em pacientes com epilepsia do lobo temporal por esclerose mesial temporal (EMT); estudar a associação entre sintoma depressivo e QV; e entre localização da lesão estrutural e sintomas depressivos. Metodologia: Setenta pacientes com EMT em tratamento ambulatorial, foram avaliados entre junho/2003 e abril/2005. Além da anamnese clínica, exames subsidiários (EEG, vídeo-EEG, RNM), avaliação neuropsicológica, psiquiátrica, todos foram submetidos à avaliação de QV que incluiu uma entrevista semidirigida, o questionário Medical Outcomes Short-Form 36 (SF-36) e o Inventário de Depressão de Beck (BDI). Resultados: Quarenta e dois pacientes (60%) eram do sexo feminino e 28 (40%) do masculino, com média de idade de 37 anos e 26 de duração da epilepsia. Em quarenta (57%) foi constatada EMT à esquerda e em 30 (43%) à direita. Quanto à situação de trabalho, 34 (49%) encontravam-se em auxílio doença/aposentados por invalidez/ou sem qualquer atividade produtiva. Trinta e um (44%) alegaram ser o trabalho a esfera mais prejudicada pela epilepsia. Trinta e nove indivíduos (56%) apresentaram sintomas depressivos com escores mais baixos no SF-36 quando comparados àqueles sem depressão. A pontuação alta do BDI, indicativa de depressão manteve uma associação significante (coeficiente de Spearman: p < 0,001) com todos os domínios do SF-36, com exceção de capacidade funcional (CF). Não houve correlação entre o lado da EMT e os sintomas depressivos. Conclusão: A presença de depressão infuencia o julgamento individual em relação a QV. O uso sistemático de instrumentos de fácil manuseio permitiria a identificação precoce de distúrbios de humor, contribuindo significativamente para a melhoria da QV de indivíduos com epilepsia. INTRODUCTION: Depressive symptoms and quality of life in people with epilepsy related to mesial temporal sclerosis Introduction: Difficulties in working, interpersonal, familial and social relationships, the perception of stigma, the discrimination by others have been associated with interictal depressive state influencing negatively the quality of live (QOL) of people with epilepsy. Depression has a high prevalence (20-55%) in epilepsies, being considered the most important factor in the judgment of the patient concerning his QOL. Objective: To evaluate the occurrence of depressive symptoms in patients with temporal lobe epilepsy due to mesial temporal sclerosis (MTS); to study the association between depressive symptoms and QOL as well as of the lateralization of the structural lesion and depressive symptoms. Methodology: 70 patients with MTS in the Outpatient Clinic were evaluated between June 2003 and April 2005. Besides the clinical anamnesis and examination all the patients had ancillary exams (EEG, video-EEG, MRI), psychological and psychiatric evaluation and QOL evaluation which was performed with a semi-structured interview, the questionnaire Medical Outcomes Short Form 36 (SF-36) and the Beck Depression Inventory (BDI). Results: 42 (60%) patients were female and 28 (40%) male. The average age was 37 and the mean epilepsy duration 26. In 40 (57%) MTS was located on the left and in 30 (42%) on the right. As to the work situation, 34 (49%) were in welfare/retired due to illness/without any productive activity. 31 (44%) stated that lack of work was the greatest burden of epilepsy. 39 (56%) presented depressive symptoms with lower scores in the SF-36 when compared to those without depression. A high score of the BDI, an indication of depression, maintained a significant association (Spearman coefficient p < 0.001) in all SF-36 domains with the exception of Functional Capacity. There was, however, no correlation between the lateralization of MTS and depressive symptoms. Conclusion: The presence of depression influences the personal judgment in relation to QOL. The systematic use of simple instruments would allow the precocious identification of humor disturbances contributing significantly to the improvement of QOL of people with epilepsy.

OBJECTIVES: Patients with temporal lobe epilepsy (TLE) and unilateral severe hippocampal sclerosis (HS) may have contralateral temporal scalp ictal onset. This has recently been called "burned-out hippocampus", which is believed to be a rare entity. In this study we report four patients with unilateral hippocampal sclerosis and contralateral ictal onset registered by scalp electrodes. We discuss the importance of such cases in presurgical evaluation of patients with TLE, as well as possible strategies used for evaluation of these particular cases. PATIENTS AND METHODS: We reviewed charts from all patients with TLE submitted to pre-surgical evaluation, which included high resolution MRI and prolonged video-electroencephalogram (video-EEG) monitoring with scalp and sphenoidal electrodes, during a three-year period (2002-2004). We looked for patients who only had seizures that were clearly contralateral in location to the atrophic hippocampus. RESULTS: Four patients fulfilled the criteria above. Two of these patients had semi-invasive video-EEG monitoring with foramen ovale (FO) electrodes, which revealed seizures originating from the temporal lobe with the atrophic hippocampus, hence confirming false lateralization in the scalp-sphenoidal EEG. These patients were submitted to surgical treatment and had favorable prognosis after surgery. CONCLUSIONS: Burned-out hippocampus syndrome may not be as rare as it was previously believed. Further studies will be necessary before one can affirm that patients with unilateral HS and scalp ictal EEG showing contralateral ictal onset may be operated without confirmation of the epileptogenic zone by invasive monitoring. In these patients, semi-invasive monitoring with FO electrodes might be an interesting alternative. RESUMO OBJETIVOS: Pacientes com epilepsia do lobo temporal (ELT) e esclerose hipocampal (EH) unilateral acentuada podem apresentar início ictal contralateral à EH no registro com eletrodos de superfície. Esta situação foi recentemente chamada de "burned-out hippocampus", que pode corresponder a uma entidade rara. Nesse estudo, nós relatamos os casos de quatro pacientes com EH unilateral marcada e início ictal contralateral à EH nas crises registradas com eletrodos de escalpo. Nós discutimos a importância desses casos na avaliação pré-cirúrgica de pacientes com ELT, bem como as possíveis estratégias utilizadas na avaliação desses pacientes. PACIENTES E MÉTODOS: Nós revisamos os dados de todos os pacientes com ELT submetidos a avaliação pré-cirúrgica, que incluiu ressonância magnética (RM) de encéfalo de alta definição e monitorização prolongada com vídeo-eletroencefalograma (vídeo-EEG) com eletrodos de superfície, em um período de 3 anos (2002-2004). Procuramos por pacientes que apresentaram apenas crises com início ictal contralateral ao lado da EH. RESULTADOS: Quatro pacientes preencheram os critérios acima. Dois desses pacientes foram submetidos a monitorização semi-invasiva com eletrodos de forame oval (FO), que mostraram crises com origem no lobo temporal com o hipocampo atrófico, confirmando assim a falsa lateralização no EEG com os eletrodos de superfície. Esses pacientes foram submetidos a tratamento cirúrgico e apresentaram prognóstico pós-operatório favorável com relação ao controle das crises. CONCLUSÃO: "Burned-out hippocampus" pode não ser tão raro quanto se acreditava antes. Mais estudos serão necessários antes que seja possível afirmar que pacientes com EH unilateral e crises com início ictal contralateral à EH no registro com eletrodos de superfície podem ser operados sem confirmação da zona epileptogência através de monitorização invasiva. Nesses pacientes, a monitorização semi-invasiva com eletrodos de FO pode ser uma alternativa.

The long-term outcome of chronic epilepsy remains largely unknown, despite a long historical experience. We report the lifelong course of epilepsy of an historical cohort of 235 subjects who were in residential care at the Chalfont Centre for Epilepsy: 122 had comprehensive post-mortem examination. The populations admitted as resident to the centre over time followed the evolution of society’s perception of epilepsy. ‘Early residents’ (before 1972) were admitted for sheltered employment, escaping stigmatization, whereas ‘later’ residents with more severe epilepsies were admitted for care. Subjects admitted before 1972 were similar to subjects followed nowadays as outpatients, whereas patients admitted later with a higher burden of disabilities are often those in residential care. This long follow-up allowed exploration of a wide spectrum of epilepsies, affecting both subjects who were otherwise healthy and those with co-morbidities. Age at death showed a bimodal distribution with an early peak of mortality between 45–50 years old, whilst the remainder had life expectancy comparable to the general population. As a group, subjects who had post-mortem examination were not significantly different from patients who did not have post-mortem examination, but post-mortem examination provided data that were otherwise unavailable. For those who had post-mortem examination, sudden unexpected death in epilepsy (SUDEP, 18% of all deaths) did not fully explain the early mortality, to which co-morbidities contributed. High seizure frequency was a significant independent predictor of early death even after excluding SUDEP (e.g. reduction in years of life for those who had >4 seizures/month compared with those who had

Startle epilepsy is a syndrome of reflex epilepsy in which the seizures are precipitated by a sudden and surprising, usually auditory, stimulus. We describe herein a girl who had been suffering with startle-induced seizures since 2 years of age. She had focal, tonic and tonic-clonic seizures, refractory to antiepileptic treatment. Daily tonic seizures led to very frequent falls and morbidity. Neurologically, she had no deficit. Interictal EEG showed slow waves and epileptiform discharges in central and fronto-central regions. Video-polygraphic recordings of seizures, triggered by stimuli, showed generalised symmetric tonic posturing with ictal EEG, characterised by an abrupt and diffuse electrodecremental pattern of fast activity, followed by alpha-theta rhythm superimposed by epileptic discharges predominantly over the vertex and anterior regions. Magnetic resonance imaging showed no abnormalities. Corpus callosotomy was performed when the patient was 17. Since surgery, the patient (one year follow-up) has remained seizure-free. Corpus callosotomy may be considered in patients with startle epilepsy and tonic seizures, in the absence of focal lesions amenable to surgery. [ Published with video sequences]

Purpose The aim of the study was to report the seizure outcome, motor skills and adaptive motor functions in a series of children and adolescents who underwent hemispheric surgery, analysing the risk-benefits of surgery. Methods The clinical course, seizure and motor function outcomes of 15 patients who underwent hemispheric surgery were reviewed. Results The mean age at surgery was 9.5, with 1–9 years follow-up. The underlying pathologies were Rasmussen encephalitis, vascular disorders, and hemimegalencephaly. All the patients presented with severe epilepsy and different degrees of hemiparesis, although motor functionality was preserved in 80% of the patients. At last follow-up, 67% were seizure free, and 20% rarely experienced seizures. Antiepileptic drugs were reduced in 60%, and complete withdrawal from such drugs was successful in 20% of the patients. The motor outcome following the surgery varied between the patients. Despite the motor deficit after surgery, the post-operative motor function showed unchanged for gross motor function in most (60%), while 27% improved. Similar results were obtained for the ability to handle objects in daily life activities. Sixty percent of the children were capable of handling objects, with somewhat reduced coordination and/or motor speed. Conclusion Pre-surgical motor function continues to play a role in the pre-surgical evaluation process in order to provide a baseline for outcome. Hemispheric surgery, once regarded as a radical intervention and last treatment resource, may become routinely indicated for refractory hemispheric epilepsy in children and adolescents, with oftentime favourable motor outcomes.

The aim of this retrospective study of a series of patients with mesial temporal lobe epilepsy (MTLE) and mesial temporal sclerosis (MTS) was to analyze the association of granule cell dispersion (GCD) with surgical prognosis, patterns of MTS and clinical data. Hippocampal specimens from 66 patients with MTLE and unilateral MTS and from 13 controls were studied. Quantitative neuropathological evaluation was performed on NeuN-stained hippocampal sections. Patients' clinical data, types of MTS and surgical outcome were reviewed. GCD occurred in 45.5% of cases and was not correlated with clinical variable. More severe neuronal loss was observed in patients with GCD. Except for MTS Type 2 - observed only in four no- GCD patients - groups did not differ with respect to the types of MTS. Surgical outcome was similar in both groups. In conclusion, GCD was associated with the degree of hippocampal cell loss, but was not a predictor of surgical outcome.

Pathology studies in epilepsy patients bring useful information for comprehending the physiopathology of various forms of epilepsy, as well as aspects related to response to treatment and long-term prognosis. These studies are usually restricted to surgical specimens obtained from patients with refractory focal epilepsies. Therefore, most of them pertain to temporal lobe epilepsy (TLE) with mesial temporal sclerosis (MTS) and malformations of cortical development (MCD), thus providing information of a selected group of patients and restricted regions of the brain. Postmortem whole brain studies are rarely performed in epilepsy patients, however they may provide extensive information on brain pathology, allowing the analysis of areas beyond the putative epileptogenic zone. In this article, we reviewed pathology studies performed in epilepsy patients with emphasis on neuropathological findings in TLE with MTS and MCD. Furthermore, we reviewed data from postmortem studies and discussed the importance of performing these studies in epilepsy populations. Estudos de patologia em pacientes com epilepsia trazem informações úteis para compreender a fisiopatologia de várias formas de epilepsia, bem como aspectos relacionados ao tratamento e ao prognóstico a longo prazo. Esses estudos são usualmente restritos a espécimes cirúrgicos obtidos de pacientes com epilepsias focais refratárias. Portanto, a maioria diz respeito à epilepsia do lobo temporal (ELT) com esclerose mesial temporal (EMT) e às malformações do desenvolvimento cortical (MDC). Desse modo, fornecem informações sobre um grupo selecionado de pacientes e sobre regiões restritas do cérebro. Raramente são realizados estudos post mortem do cérebro inteiro em pacientes com epilepsia; entretanto, eles podem prover informações relevantes sobre patologia cerebral, permitindo a análise de áreas além da zona epileptogênica putativa. Neste artigo, foram revisados estudos de patologia feitos em pacientes com epilepsia, com ênfase nos achados neuropatológicos em ELT com EMT e MDC. Foram revisados, ainda, dados de estudos post mortem e discutiu-se a importância da realização desses estudos em populações de pessoas com epilepsia.

This study aimed to analyze the impact of ictal dystonic posturing (DP) in postoperative seizure outcome and to assess the influence of DP in generalized tonic-clonic seizure (GTCS) occurrence during video-EEG monitoring of patients with temporal lobe epilepsy with mesial temporal sclerosis. The impact of DP on surgical outcome remains controversial. Moreover, DP has been recently associated with brain networks avoiding GTCS occurrence. Five hundred twenty-seven seizures of 171 patients who were submitted to standard anterior temporal lobectomy (ATL) between 2002 and 2010, with at least one year of post-surgical follow-up, were retrospectively analyzed and classified as with or without DP and as evolving or not to GTCS. The ictal semiologic correlates of DP, timing elapsed since precedent seizure and antiepileptic drug (AED) intake before each seizure were evaluated. Seizure outcome after ATL was assessed according to Engel's scale. Fifty-eight out of 171 patients (34%) exhibited ictal DP, of which 91.5% were always unilateral and contralateral to the operated side. DP was related to shorter seizures (p=0.007) and a much lower likelihood of the seizure evolving to GTCS (p=0.001), even during AED withdrawal (p=0.002). There was no association between DP and prognosis regarding seizure control as the result of the surgical resection, either in patients with shorter or in those with longer period of follow-up. Our data support the hypothesis that DP reflects a brain network activation that helps avoid GTCS, even during AED withdrawal.
