Objective: The purpose of this study was to assess the effects of treatment with selective serotonin reuptake inhibitors (SSRIs) on the risks of gestational hypertension and preeclampsia. Method: The authors analyzed data from 5,731 women with nonmalformed infants and no underlying hypertension who participated in the Slone Epidemiology Center Birth Defects Study from 1998 to 2007. Gestational hypertension was defined as incident hypertension diagnosed after 20 weeks of pregnancy, with and without proteinuria (i.e., with and without preeclampsia). The risks of gestational hypertension and preeclampsia were compared between women who did and did not receive SSRI treatment during pregnancy. Relative risks and 95% confidence intervals (CIs) were estimated using the Cox proportional hazards model, adjusting for prepregnancy sociodemographic, lifestyle, reproductive, and medical factors. Results: Gestational hypertension was present in 9.0% of the 5,532 women who were not treated with SSRIs and 19.1% of the 199 women who were treated with SSRIs. Among women who received treatment, gestational hypertension was present in 13.1% of the 107 women who received treatment only during the first trimester and in 26.1% of the 92 women who continued treatment beyond the first trimester. The occurrence of preeclampsia was 2.4% among women who were not treated with SSRIs, 3.7% among women who were exposed to SSRIs only during the first trimester, and 15.2% among women who continued SSRI treatment beyond the first trimester. Relative to women who did not receive treatment, the adjusted relative risk of preeclampsia was 1.4 for women who discontinued treatment and 4.9 for women who continued treatment. Conclusion: SSRI exposure during late pregnancy--whether a causal factor or not might--identify women who are at an increased risk for gestational hypertension and preeclampsia. Further investigation is needed in order to separate the effects of treatment with SSRIs from those of underlying mood disorders.

Preeclampsia, gestational hypertension, and unexplained intrauterine growth restriction having similar determinants and consequences are studied comparing determinants and perinatal outcomes associated with obstetric conditions. The results reveal that preeclampsia and gestational hypertension have related condition, both increasing the risk of morbidity and morality.

Pregnant women who have excessive amounts of human chorionic gonadotropin in their blood are more likely to have an adverse pregnancy outcome such as miscarriage, low birth weight babies, premature birth, fetal growth restriction, and hypertension. This was the conclusion of a study of 3,870 pregnant women. Pregnant women with high blood levels of human chorionic gonadotropin should receive extensive prenatal care.

In this study, the authors related blood lead concentrations to Apgar scores, birth weight, gestational age, small-for-gestational age, and hypertension in pregnancy (HIP)/toxemia. Data and blood were collected 4 times during pregnancy from 705 women, aged 12-34 yr. Blood lead concentrations, measured by atomic absorption spectrophotometry, were related to reproductive outcomes, abstracted from medical records. Average blood lead concentrations were 1.2 [micro]g/dl (standard error = [+ or -] 0.03). Maternal blood lead concentrations were related significantly to HIP/toxemia--before and after adjusting for age, calcium intake, and race/ethnicity (p < .03). Longitudinal regression analyses revealed that blood lead concentrations in women with HIP/toxemia changed by 0.02 [micro]g/dl for every 0.01 [micro]g/dl change in women without HIP/toxemia. Maternal blood lead concentration and its change were not significantly associated with other reproductive outcomes. Women who smoke but who quit when they learn they are pregnant do not have a lower risk of hypertension or preeclampsia during pregnancy. This is the conclusion of a study of 4,589 women, which contradicts other studies showing a reduced risk of hypertension and preeclampsia in pregnant women who smoke during pregnancy.

Smoking appears to lower the risk of hypertension in pregnancy even if the woman is a former smoker, according to a study of 9,651 pregnant women. Current and former smokers also had a lower risk of preeclampsia, a serious complication of pregnancy.

Insulin resistance may be a factor in hypertensive disorders of pregnancy. Gestational diabetes mellitus is associated with increased risk for preeclampsia or other hypertensive disorders during pregnancy, although not at a statistically significant level. Likelihood of preeclampsia also is positively correlated with the plasma glucose level one hour after a 50-g oral glucose challenge. Preeclampsia was not prevented by calcium supplementation.

Women with pregnancies complicated by placenta previa may have a lower risk for pregnancy-induced hypertension. Placenta previa is the attachment of the placenta in the lower portion of the uterus, where the placenta may precede the baby into the birth canal. Over a 14-year span, of 121,082 pregnancies studied, 416 were complicated by placenta previa. Women with this complication were half as likely to develop high blood pressure during pregnancy than those without, even after allowing for influencing factors, such as the shorter gestation period usually seen due to placenta previa.

Elevated blood levels of second-trimester chorionic gonadotropin may be associated with a higher risk of pregnancy-induced hypertension. Hypertension is persistent high systolic blood pressure. Among 549 women whose blood levels of chorionic gonadotropin were measured in their second trimester of pregnancy, 180 had elevated blood levels and 369 had normal blood levels (control group). The risk of pregnancy-induced hypertension was significantly higher in the women with elevated blood levels than in those with normal blood levels. Women with elevated blood levels also had a higher risk of pregnancy-induced hypertension with proteinuria. Proteinuria is the presence of protein in the urine.

Women who develop asthma during pregnancy may have a higher risk of pregnancy-induced hypertension than those who do not. Both of these conditions may be caused by a factor that affects smooth muscle. Among 24,115 women who delivered a live or stillborn infant between Jan 1987 and Dec 1991, 136 developed asthma during pregnancy and 1,435 had a history of asthma. Ninety-two women developed severe pregnancy-induced hypertension and 1,307 developed moderate pregnancy-induced hypertension. An association was found between the development of asthma during pregnancy and pregnancy-induced hypertension. The incidence of asthma was higher among women with severe pregnancy-induced hypertension than among those with moderate pregnancy-induced hypertension.

The influence of increased prepregnancy body mass index (BMI) on pregnancy outcome in women with mild gestational hypertension remote from term is studied. In patients with mild gestational hypertension remote from term, an obese prepregnancy BMI was associated with similar rates of progression to preeclamsia.

A study is conducted to determine the demographics, outcomes and treatment of hypertensive disorders that are diagnosed in the postpartum period. It is suggested that efforts should be directed at the continued monitoring, reporting ad evaluating of the symptoms of preeclampsia during the postpartum period.

The evidence that calcium plays a role in the etiology, prevention, and treatment of pregnancy-induced hypertension (PIH) is reviewed. The precise factors involved in the pathogenesis of PIH are unclear, but several alterations in calcium metabolism have been identified. Epidemiologic data suggest an inverse correlation between dietary calcium intake and incidence of PIH. Although evidence suggests a possible beneficial effect of supplemental calcium, contradictions persist in clinical trials of pregnant women. Presently, there is insufficient evidence to support routine calcium supplementation of all pregnant women. However, high-risk groups, such as pregnant teens, populations with inadequate calcium intake, and women at risk of developing PIH, may benefit from consuming additional dietary calcium.
