Aim: The extent of the spread of inflammation determines the severity of acute pancreatitis (AP). Methylated arginine residues (MAR), a type of inflammatory mediator, reduce nitric oxide levels and cause vasoconstriction-induced endothelial damage. This study aimed to investigate MAR and related amino acids during acute pancreatic inflammation. Material and Method: This prospective, quasi-experimental study was conducted with patients diagnosed with AP and an age-matched control group. The patient samples were taken during the diagnosis and recovery time, whereas during the study for the control group. Mainly, Asymmetric dimethylarginine (ADMA), Arginine (ARG), Citrulline (CIT), and related chemicals were studied via a mass spectrometer. Results: A total of 30 patients with AP (mean age=53.3±17.8) and 30 controls (mean age=53.4±18.0) were included in the study. All patients were identified as non-severe (n=8) and severe (n=22). A decrease was detected in the patients' ADMA levels compared to the control group (p=0.01). MAR did not differ concerning disease severity (p > 0.05). However, MAR levels decreased higher in patients with diabetes or chronic kidney disease (CKD). Between the two samplings, the ARG level and ARG to ADMA ratio increased, while the MAR and CIT to ARG ratio decreased. Conclusion: Our results showed that MAR levels decreased with AP recovery. The start of a decrease in the high-level blood MAR may indicate the healing of pancreatic inflammation. AP inflammation may be more destructive in patients with diabetes or CKD.

Objective: We investigated the importance of netrin-1 levels in Ulcerative Colitis (UC) in clinical activity of the disease, and its association with other proinflammatory cytokines IL-6 and TNF-alpha.Materials and Methods: This study was a case-control study and included 67 patients with UC (36 activated, 31 in remission) and 50 healthy controls. Introduction: In recent years, an increase in the use of herbal supplements, natural products, and traditional medicines has been observed. These herbs and herbal products have been associated with complications, such as liver injury, causing high mortality and morbidity. Case Report: A 51-year-old female presented to the emergency department with a 2-week history of abdominal pain, vomiting, and nascent jaundice. In the detailed history of the patient, the daily consumption of Laurus nobilis tea was reported for 1 month at home. On physical examination, she was icteric and ill-looking. Mild right upper quadrant discomfort with tenderness was noted, but no rash or hepatosplenomegaly was observed. The laboratory tests results were as follows: serum AST/ALT ratio 1.2 (908/744 U/L), gamma-glutamyl transferase 63 U/L, ALP 124 U/L, bilirubin (total/direct) 27.0/14.0 mg/dL, and lactate dehydrogenase 420 U/L. Prothrombin time international normalized ratio was 2.77. The gastroenterology department considered fulminant hepatitis in the patient and recommended liver transplantation. The patient died while preparations for the transplant were ongoing. Conclusion: Many herbal products are known to be hepatotoxic. Laurus nobilis is a commonly consumed herbal product, and the potential of this herb to cause hepatotoxicity should be considered.

A general “line-line” (LL) method is proposed for propagation constant determination (γ+ and γ−) of nonreciprocal networks using multiple reference networks. The new formalism provides flexibility in implementation of LL methods so that not only more accurate γ+ (and γ−) could be retrieved but also the effect of inaccurate length information of the non-reciprocal network on γ+ (and γ−) evaluation could be suppressed. For validation of the method, γ+ and γ− of a microwave phase shifter (a nonreciprocal network) are determined using uncalibrated scattering parameter measurements at X-band (8.2–12.4 GHz). Two different statistical analyses based on normalized root-mean-square-error and goodness of fit and based on coefficient of variation and confidence interval are individually performed to evaluate the performance and accuracy of our method. © 2022

The purpose of this study is to investigate the factors affecting the acceptance and use of the APTS by its potential users. To achieve this, an integrated and expanded user acceptance model is introduced to explain the factors affecting the Behavioral Intention to use the APTS. A total of 316 surveys were conducted, from 275 participants by online surveys and 41 participants by face to face interviews. With the data that are acquired from Public Transportation (PT) users in Istanbul, the Unified Theory of Acceptance and Use of Technology 2 (UTAUT2) Model is modified and Performance Expectancy, Effort Expectancy, Social Influence, Facilitating Conditions, Hedonic Motivation, Price Value, Habit, Trust and Safety, Perceived Usefulness, Perceived Risk and Behavioral Intention constructs are utilized in the model. Age, gender and frequency of PT usage are included in the model to examine the moderating effect on the constructs while the model is tested with Structural Equation Modeling (SEM). The proposed model explains the 72 % of variances in this study which proves that the model has stronger predictive power compared with the previous Technology Acceptance Model (TAM) and UTAUT models. Performance Expectancy, Social Influence, Habit, and Trust and Safety constructs are concluded to have significant positive effects on Behavioral Intention. Public usage and acceptance are necessary for the complete utilization of APTS. The findings of these studies are hoped to guide the decision-makers of the PT industry to understand the factors affecting the use and acceptance of APTS and take the actions accordingly.

Surface curing effect of hardened engineering cementitious mortar (ECM) samples with distinct water-to-binder ratios ( w / b ) is analyzed by recording amplitude of reflection response | Γ | or conductance term g measured by a microwave open-ended waveguide probe. Frequency dependencies (between 3.95–5.85 GHz) of | Γ | and g measured from various surfaces (top, bottom, and lateral) of ECM samples with w / b = 0 . 2 and 0.3 are presented at the 720th days of curing (hardened ECM). In addition, their temporal dependencies (over 1–9 and 720–815 days) are presented for the frequency value of 4.56 GHz. From our analysis, contrary to top and lateral surface behaviors, it is noted that crossover between g values measured from bottom surfaces of ECM samples with w / b = 0 . 2 and 0.3 delays for a long time (around 550 days). Propagation-related studies should take into account reflection profiles varying with wall height for accurate prediction.

Background/aim Inflammatory bowel disease (IBD) mainly encompass two entities called ulcerative colitis (UC) and Crohn's disease (CD), both of which are chronic, progressive and, inflammatory conditions of the gastrointestinal tract. Various indicators and non-invasive markers have been sought and used in IBD patients to help assessing disease activity and treatment effectiveness although none of them are proven to yield definite results in full correlation with the clinical, endoscopic, and histopathological examinations. The aim of the current study was to investigate the relationship of serum neutrophil gelatinase-associated lipocalin (NGAL), asymmetric dimethylarginine (ADMA), and symmetric dimethylarginine (SDMA) levels with disease type and activity and to assess their potential use in establishing diagnosis and activity status of IBD, namely UC and CD. Materials and methods A total of 111 IBD patients with determined active and inactive disease periods and 70 matched controls were recruited. Serum NGAL levels of the patients and the control group were measured using commercially available ELISA kits. ADMA and SMDA levels were measured by high performance liquid chromatography. Results The IBD group had significantly higher serum levels of NGAL (p = 0.001), ADMA (p = 0.0001), and SDMA (p = 0.0001) in comparison to the control group. Likewise, serum NGAL, ADMA, and SDMA levels were significantly higher in the active IBD group compared to the inactive IBD group (p = 0.0001). Active UC and active CD patients similarly had significantly higher levels of serum NGAL, ADMA, and SDMA than the respective levels in inactive UK and inactive CD patients (p = 0.0001). Conclusion Serum NGAL, ADMA and SMDA levels are increased in patients with IBD, and serum NGAL, ADMA and SMDA concentrations are significantly higher in active IBD patients than inactive IBD patients. Our results suggest these biomarkers may serve in estimating IBD activity and severity.

A methodology relying on relating terms of a two-port error network to the scattering (S-) parameters of a two-port network or device is applied to extract its full S-parameters. The new methodology has only one sign ambiguity problem (two solutions) in evaluation of S-11 (and thus S-22) while the similar methodology in the literature has two sign ambiguity problems (4 solutions). This ambiguity problem of our method is shown to be eliminated by applying an approach based on continuity of the argument of S-11 in the frequency domain. On the other hand, our methodology, as compared with the thru-reflect-line calibration technique, does not necessitate usage of any reflection standard in determining full S-parameters of a two-port network or device. Finally, it gives information about error networks. For its validation, S-parameters of a microwave phase shifter and a polyethylene sample flushed with the left/bottom terminal of the coaxial cell were extracted.

The aim of the present study was to investigate the presence of HLA-DQ2/DQ8 in patients with celiac disease, their first-degree relatives, and healthy community. Materials and Methods: HLA-DQ2/DQ8 analysis was performed in adult patients with celiac disease >= 18 years old (94 patients), their first-degree relatives (89 people), and healthy group (102 individuals). Anemia, osteoporosis, and diarrhea were interrogated in the celiac patient group and also anti-tissue transglutaminase, anti-endomysium, and anti-gliadin antibodies were recorded. Results: There was a significant relationship between HLA-DQ2/DQ8 presence in all groups, and the distribution of HLA-DQ2/DQ8 in all groups was different (p=0.000). No statistically significant correlation was found between the HLA tissue groups and diarrhea (p=0.087), osteoporosis (p=0.215), anemia (p=1.000), tissue transglutaminase antibodies (p=0.295), anti-gliadin antibodies (p=0.104), and anti -endomysium antibodies (p=0.243) in the celiac patient group. Stomach cancer is an increasing incidence of cancer both in the World as well as in Turkey. The most common method used worldwide for gastric cancer diagnosis is endoscopy. However, definitive diagnosis is made with endoscopic biopsy results. Diagnosis with endoscopy is a very specific and sensitive method. With high-resolution endoscopy it is possible to detect mild discolorations, bulges and structural changes of the surface of the mucosa. However, because the procedures are performed with the eye of a doctor, it is possible that the cancerous areas may be missed and / or incompletely detected. Because of the fact that the cancerous area cannot be completely detected may cause the problem of cancer recurrence after a certain period of surgical intervention. In order to overcome this problem, a computerized decision support system (CDS) has been implemented with the help of specialist physicians and image processing techniques. The performed CDS system works as an assistant to doctors of gastroenterology, helping to identify the cancerous area in the endoscopic images of the scaffold, to take biopsies from these areas and to make a better diagnosis. We believe that gastric cancer will be helpful in determining the area and biopsy samples taken from the patient will be useful in determining the area. Research data demonstrating nutritional habits of functional dyspepsia (FD) patients are very limited. This is the first study to evaluate nutritional habits in FD subgroups according to Rome III criteria. Our aim was to evaluate nutritional habits of FD patients and determine the food items that may provoke a dyspepsia symptom.A total of 168 adults with FD and 135 healthy control subjects participated in the study. FD subjects were divided into epigastric pain syndrome (EP-FD), postprandial distress syndrome (PS-FD), mixed (MX-FD) subgroups according to Rome Criteria III. Subjects completed a questionnaire that included a short-form food frequency questionnaire. Furthermore, subjects were asked to list the food items that were causing a dyspepsia symptom.Functional dyspepsia subjects had a slightly higher BMI (26.1 ± 4.97 kg/m(2)) than control subjects (24.6 ± 4.08 kg/m(2)). The most common symptom triggering foods among all the FD groups were fried and fatty foods (27.1%), hot spices (26.4%), and carbonated drinks (21.8%). In FD subgroups, carbonated drinks were more likely to cause a symptom in PS-FD group (37.3%) than MX-FD (25.7%) and EP-FD (22.1%) groups. There was no difference in frequency of main meals and snacks among any of the groups.Fatty and spicy foods and carbonated drinks were the most common symptom triggering food items in FD group. In subgroups, carbonated drinks and legumes were more likely to cause a symptom in PS-FD. Removing these food items during the course of treatment might help alleviate the symptoms.

Introduction Endometrial polyps (EPs) and colorectal polyps (CPs) are common disorders and the incidence of both increase rapidly with aging. CPs are focal lesions and incidence increases with age. Aim In this study, we aimed to analyze retrospectively the relationship between the EPs and CPs sharing similar clinical and genetic factors in their etiopathogenesis. Materials and Methods This study was retrospectively performed between 2010 and 2013 and it included patients diagnosed to have eEPs. The study group and the control group consisted of patients who were diagnosed with or without EPs and who underwent colonoscopy at the same period. Results The study group was formed by 57 patients with diagnosis of EP who underwent colonoscopy during the same period. The control group consisted of 71 patients without EP examined with colonoscopy. Among 128 patients assessed in this study, 24 were diagnosed with CPs, all of which were adenomatous polyps smaller than 1 cm. No hyperplastic or inflammatory polyps were diagnosed. While 18 of 57 patients with EPs had CPs, 6 of 71 control subjects had CPs. Hence, the risk of having CP was 5 times greater in patients with EP compared to those without ( P Conclusion The present study demonstrated that the prevalence of CPs increased significantly in postmenopausal patients with EPs. Recommending colonoscopy to these patients irrespective of age may be beneficial for detecting more CPs and preventing colorectal cancer.

Image processing is a field of which its popularity increases and continues and, that grows dynamically with new technologies. Nowadays, image processing finds itself in use in almost every field. One of these uses is undoubtedly in the field of medicine, where diagnosis and treatment planning are made from images and, which is constantly changing with newly developed techniques. Of course, the most important factor in using this so widely in the medical field is the acquisition of images on every medical field. With the help of these images, the complaints can be seen more easily and the doctor can follow a path in the treatment of the disease. In our study, we used the Region Growing segmentation method to detect the intestinal mass. This study compares the area determined by the specialist with the area obtained with the segmentation process and, it is seen that the created software system can be used as an auxiliary system to specialist doctors.

The purpose of this study was to noninvasively estimate disease activity and liver fibrosis in patients with nonalcoholic fatty liver disease (NAFLD) using anthropometric and biochemical characteristics and the C-methionine breath test (MeBT).A total of 164 patients with histologically proven NAFLD and 56 healthy controls were included in the study. Anthropometric and biochemical analyses and the MeBT were performed on all patients and controls.BMI; waist circumference; waist-hip ratio; transaminase, lipid, γ-glutamyl transpeptidase (GGT), glucose, and insulin levels; and insulin resistance were significantly higher in patients with NAFLD than in controls. The GGT level and the MeBT were independent predictors of nonalcoholic steatohepatitis (NASH). Fibrosis was correlated with GGT, bilirubin, cholesterol, and insulin levels, and the MeBT, but the test was the only independent predictor of significant fibrosis. Patients with simple steatosis had similar MeBT values as controls. The MeBT values were significantly lower in NASH and NASH-cirrhosis patients (P0.001) compared with simple steatosis patients and controls. Patients with advanced fibrosis (F2-3) had significantly lower MeBT values than patients with mild fibrosis (F0-1; P0.001). The area under the receiving operating characteristic curve for NASH and advanced fibrosis was estimated to be 0.95 in the total cohort.This study indicates that anthropometric and biochemical parameters are insufficient for estimating the presence of NASH or the fibrosis stage. However, the MeBT is a suitable noninvasive method for accurately predicting which patients suffer from simple steatosis, NASH, or NASH-cirrhosis.

Background/Aims: To provide a new mathematical formula to predict liver fibrosis in patients with chronic viral hepatitis. Materials and Methods: Patients with chronic hepatitis B and C who underwent liver biopsy at different centers were included in this study. Chronic hepatitis B was defined as immunopositivity for the hepatitis B surface antigen for at least 6 months, and chronic hepatitis C was defined as positivity for HCV RNA for at least 3 months. The histo logical features were evaluated by the histological activity index and fibrosis. Results: In total, 1299 patients were included in the study. The distribution and the mean of the parameters of the patients were as follows: 1009 patients with chronic hepatitis B with a mean age of 45±13/years [emale/male (F/M)=47.5/52.5%] and 290 patients with hepatitis C with a mean age of 52±10.3/years [F/M=61/39%]. When the cut-off value of the REAL TEST formula“[(age x pT x AST)/(PLT/1000)]/100” in patients with hepatitis B was deter mined to be ≥1.37, it was found that it could predict fibrosis with 79% specificity, 78% sensitivity, 85% negative predictive value (NPV), and 70% positive predictive value (PPV) (area under the curve (AUC)=0.852, 95% CI:0.820.87). When the cut-off value of the REAL TEST formula in patients with hepatitis C was determined to be ≥1.99, it was found that it could predict significant fibrosis with 87% specificity, 90% sensitivity, 94.4% NPV, and 79.4% PPV (AUC:0.95, 95% CI:0.93-0.98) Conclusion: The REAL TEST formula results correlated with the pathological findings and may be a useful method for the evaluation of patients with chronic hepatitis B and C.

Previous studies suggest that serum IGF-1 is higher in women with polycystic ovary syndrome (PCOS). The ophthalmologic effects of IGF-1 excess have not yet been investigated in women with PCOS. The aim of the current study is to compare the corneal thickness of patients with PCOS and those of healthy subjects. Forty three patients with PCOS and 30 age-matched and gender-matched healthy individuals were enrolled in this cross-sectional study. Central corneal thickness (CCT) was measured in patients with PCOS and in healthy individuals with an ultrasound pachymeter. IGF-1 values were also determined in the study group. Women with PCOS had significantly higher levels of IGF-1 and homeostasis model assessment (HOMA-IR) levels than the control group. Right and left CCT measurements were higher in the PCOS group than in the control group. A positive correlation between IGF-1 and right and left CCT was identified in both groups. In multiple linear stepwise regression analyses, IGF-1 independently and positively associated with HOMA-IR in women with PCOS. A correlation between total testosterone and CCT was identified in the whole group. In multiple stepwise regression analyses, total testosterone independently and positively associated with left central corneal thickness in the whole group. These findings indicate that PCOS has target organ effects on the eye. Consequently, it can change central corneal thickness. Higher IGF-1 levels seem to be the main causes of increased corneal thickness. Insulin resistance in PCOS is one of the underlying causes and promotes increase in IGF-1. We suggest a careful and detailed corneal evaluation in PCOS patients to prevent the potential risk of increased CCT, in addition to the already-known complications.

Both ulcerative colitis and Crohn's disease are forms of inflammatory bowel disease (IBD), which is characterized by chronic, progressive inflammation of the gastrointestinal tract. Recent studies have shed new light on the importance of inflammation in the pathogenesis of arterial stiffness.This study aimed to evaluate the relationship between pulse wave velocity (PWV) measurement and biochemical parameters in inactive and active IBD patients without cardiovascular risk factors and perform a comparison with the control group.We enrolled 102 IBD patients without cardiovascular risk factors and 74 matched controls, and evaluated each patient in active and inactive disease periods. All patients completed a standard questionnaire form and we assessed various laboratory parameters. We carried out vascular measurements using a Mobil-O-Graph 24-h pulse wave analysis monitor, an automatic oscillometric device.Although cardiovascular risk factors, such as total cholesterol and low-density lipoprotein cholesterol, were significantly lower (P0.05) in IBD patients than the controls, 24 h, day and night PWV values, erythrocyte sedimentation rate, C-reactive protein, insulin, homeostasis model assessment of insulin resistance, and homocysteine were higher in patients with active and inactive IBD than the controls (P0.05). Multiple linear regression analysis showed that PWV was correlated positively with age and duration of IBD.This study showed increased PWV, homocysteine, erythrocyte sedimentation rate, C-reactive protein, insulin, and homeostasis model assessment of insulin resistance in patients with active and inactive IBD and provides evidence of the potential contribution of inflammation and inflammation-related factors toward arterial stiffening independent from conventional cardiovascular risk factors.

Hypersecretion of PTHrP is a relatively common cause of malignancy-related hypercalcemia. However, there is only one case report of letrozole induced hypercalcemia. A 52-year-old female patient was referred to our clinic because of the recent discovery of hypercalcemia (11.0 mg/dL). The patient had a history of left breast carcinoma. She had started a course of letrozole (aromatase inhibitor; 2.5 mg dose/day) ten months earlier. Patient’s parathyroid hormone-related protein levels were normal and a bone scintigram revealed no evidence of skeletal metastasis. Other potential causes of high calcium levels were ruled out. We recognized that, when letrozole was taken at one dose daily (2.5 mg), she had recurrent hypercalcemia. Our experience suggests that letrozole may precipitate hypercalcemia in a patient with breast cancer.

Primary ovarian insufficiency (POI) is not only a gynecological problem but also has serious effects on women's health such as changes in hormone levels that can trigger fluctuations in blood sugar level and inflammation status. The present study was designed to determine vitamin D, copper, zinc, metabolic parameters [insulin, homeostasis model of assessment-insulin resistance (HOMA-IR)], inflammation parameters such as procalcitonin and high sensitivity C reactive protein (hs-CRP), and lipid profile in POI patients and control subjects with normal menstrual cycles. A total of 43 patients with nondiabetic POI were studied in order to evaluate and compare the findings with those of the control group, which comprised 33 women with normal menstrual cycles. The women with POI had higher levels of serum copper, serum insulin, glucose, LDL-cholesterol, total cholesterol, HOMA-IR, hs-CRP, and procalcitonin, whereas serum vitamin D and zinc levels were lower compared with the healthy control group. Follicle-stimulating hormone (FSH) levels were positively correlated with insulin, glucose, HOMA-IR, hs-CRP, procalcitonin, and copper and negatively correlated with vitamin D and zinc levels. In multivariate statistic analyses with body mass index and FSH as dependent variables, FSH was positively associated with copper and HOMA-IR negatively with vitamin D levels. The present study demonstrated that women with POI have traditional risk factors for diabetes mellitus, including lower levels of vitamin D, whereas higher levels of copper and HOMA-IR.

Background/Aims: Chronic gastritis progression is a multistep process of atrophy, intestinal metaplasia (IM), and dysplasia, which may lead to invasive carcinoma. In this study, we identified an association of colonic polyps with gastric IM in patients undergoing colonoscopy. Materials and Methods: This retrospective case-control, cross-sectional study was conducted in a tertiary-care institution in Turkey. Pathology and endoscopy reports were reviewed. The study group comprised 400 patients with colonic adenomatous polyps, and the control group comprised 360 patients without colonic adenomatous polyps on colonoscopy. OBJECTIVE Celiac disease (CD) is a lifelong, chronic, immune-mediated, inflammatory small bowel disorder, precipitated by exposure to dietary gluten and related proteins in genetically predisposed individuals. Recent studies have shed new light on the importance of inflammation in the pathogenesis of arterial stiffness. The aim of this study is to investigate the prevelance of Helicobacter Pylori ( H. pylori ), the relationship between helicobacter pylori prevelance and age-gender-histo-pathological findings. Material and Methods: Data of the patients, whom gastroscopy was performed between January 2012 – June 2013 secondary to any occasion and biopsy specimens were obtained from antrum-corpus, were investigated retrospectively. H. Pylori prevelance was determined at general patient group and subgroups which were allocated according to gender and age intervals. Results: Mean age of the patients was 49.3 ± 16.5. Pylori was negative at 1099 (60.1%) of the patients, it was positive at 730 (39.9%) of the patients, 314 male (40.41%) and 416 female (39.13%). H. Pylori prevelance was similar at both genders . Positivity of H. Pylori was mild at 57%, moderate at 30% and severe at 13% of the patients. At age allocated subgroups; H. Pylori positivity was 49.8%, 45.7%, 38.5%, 29.7% respectively at patients between 14–30, 31–45, 46–60 and 61–95 ages. Prevelance of H. Pylori was significantly higher at patients below the age of 45 (p=0.001). H. Pylori positivity was 47.8%, 40.7%, 27.0%, 18.5% and 6.1% respectively at patients with histo-pathological diagnosis of non-atrophic gastritis, atrophic gastritis, intestinal metaplasia and normal histology. Conclusion: H. Pylori prevelance is 39.9% at patients from Konya region, prevelance is similar at both genders, prevelance of H. Pylori is statistically significant at patients below the age of 45. Gerec ve yontem: Ocak 2012- Haziran 2013 doneminde, endoskopi unitesinde herhangi bir nedenle gastroskopi yapilan ve antrum-korpus biyopsisi alinan hastalar geriye donuk olarak incelendi. H. pylori sikligi genel, cinsiyet ve yas gruplarina ayrilarak belirlendi. Bulgular: Calismaya alinan hastanin ortalama yasi 49.3 ± 16.5 idi. Sonuc: Konya bolgesinde H .pylori sikligi %39,9’ olup cinsiyetler arasinda anlamli fark yokken 45 ve alti yas hastalarda H.pylori sikligi anlamli derecede yuksek bulundu (p=0.001). WOS: 000322923000009 PubMed: 23551920 Background: Several noninvasive diagnostic tests based on the detection of Helicobacter pylori stool antigen (HpSA) have been developed. The aim of the study was to compare the diagnostic accuracy of 5 HpSA tests-2 monoclonal enzyme immunoassay tests (EIAs: the Premier Platinum HpSA Plus test and Helicobacter pylori Antigen (Hp Ag) test) and 3 rapid immunochromatographic assay (ICA) tests (the ImmunoCard STAT! HpSA test, one step HpSA test, and H. pylori fecal antigen test)-for diagnosing H. pylori infection in adult patients with dyspeptic symptoms before eradication therapy. Materials and Methods: A total of 198 patients with dyspeptic symptoms were included in the study. A gastric biopsy was collected for histopathology and rapid urease testing. Stool specimens for HpSA testing were also collected. Patients were considered H. pylori positive if two invasive tests (histological and rapid urease tests) were positive. Results: The sensitivity and specificity were 92.2% and 94.4%, respectively, for the Premier Platinum HpSA Plus test; 48.9% and 88.9%, respectively, for the HP Ag test; 86.7% and 88.9, respectively, for the One Step HpSA test; 68.9% and 92.6%, respectively, for the ImmunoCard STAT! HpSA test; and 78.9% and 87%, respectively, for the H. Pylori fecal antigen test. Conclusions: The Premier Platinum HpSA Plus EIA test was determined to be the most accurate stool test for diagnosing H. pylori infections in adult dyspeptic patients. The currently available ICA-based tests are fast and easy to use but provide less reliable results.

Autoimmune hepatitis (AIH) and primary biliary cirrhosis (PBC) associated with Evans syndrome; combination of autoimmune hemolytic anemia (AIHA) and immune thrombocytopenic purpura (ITP) has rarely been reported. We report the case of a 53-year-old patient who presented with weakness, myalgia, arthralgia, shortness of breath and purpura. Initial laboratory investigations revealed liver dysfunction, anemia and thrombocytopenia. Anti-nuclear (ANA) and antimitochondrial M2 (AMA M2) antibodies were positive. Diagnose of PBC-AIH overlap was made by clinical, serological and histological investigations. AIHA and ITP was identified with clinical-laboratory findings and bone marrow puncture. She was treated with IVIG followed by prednisolone and ursodeoxycholic acid. Hemoglobin-thrombocytes increased rapidly and transaminases improved at day 8. We have reported the first case in the literature with AIH-PBC overlap syndrome concurrent by ITP and AIHA which suggest the presence of shared genetic susceptibility factors in multiple autoimmune conditions including AIH, PBC, ITP and AIHA.
