East Africa is particularly vulnerable to weather extremes, with severe weather linked to thousands of deaths per year. Improved forecasts of convective events in this region are urgently needed, from both nowcasting and numerical weather prediction models. Improving these forecasts requires further knowledge of convection in this region. This study aims to improve understanding of convective events in East Africa, based on a six‐year climatology of convective life cycles and the associated precipitation. Convective systems are identified as contiguous areas of cold cloud in geostationary satellite measurements over East Africa. A tracking algorithm is used to trace the evolution of the properties of these systems through time and space. Matching the systems to surface precipitation obtained from satellite microwave observations provides insight into how the life cycles of these systems relate to precipitation at the surface. Over the region as a whole, 59% of the accumulated precipitation can be attributed to the tracked convective systems. The majority (81%) of heavy precipitation events (≥$$ \ge $$10 mm·$$ \cdotp $$hr−1) are attributable to convective systems, while light rainfall events (<$$ < $$1 mm·$$ \cdotp $$hr−1) are not (1.8%). Most of the tracked convective systems have an area less than 400 km2 and last for less than an hour. However, the less frequent larger longer‐lived systems produce the majority of the regional accumulated precipitation. Composite life cycles of the tracked systems show rapid intensification and the heaviest precipitation initially, followed by a steady increase in area and weakening in intensity before the system decays. Finally, the Madden–Julian oscillation, which plays a key role in intraseasonal rainfall variability in the region, is also linked to the amount of rainfall due to convective systems through changes in the frequency and properties of these systems. Ballantyne's call for 'reality-orientedness' as a core concept of intellectual humility (IH), though laudable, is too general to unify the field. Rather than pursuing an unlikely consensus, what will likely be more successful in moving the field forward will be greater clarity about how our culturally-based ideologies − discussed as 'worldviews' − shape the understanding, experience, and manifestation of IH. Failing to account for these differences, which has heretofore characterized much IH research, leads to problematic results in the existing IH empirical literature. The focus here is on religious worldviews, where the disregard of worldview is surprising given that humility, including intellectual humility, is consistently valued in major world religions. A worldview approach as a way forward for empirical research is illustrated by discussing how two religious/philosophical traditions − a theistic Christian tradition, and a Buddhist tradition − might conceptualize IH. Distinguished by their penetrance within a population, antibiotic tolerance and persistence are superficially similar phenomena by which growth-restricted bacteria survive treatment with bactericidal antibiotics. Owing to their apparent similarity, it is often assumed that the same physiological states and molecular mechanisms underlie the ability of individual antibiotic tolerant and persistent bacteria to survive treatment. Experimentally, antibiotic persistence is an extremely challenging phenomenon to study due to both its transience and the co-existence of persisters with non-persisters in the population of interest. In contrast, antibiotic tolerance operates at the whole population level as a result of bacteria acquiring genetic mutations or encountering environmental conditions that result in growth restriction. Therefore, studying antibiotic tolerance is often used as a convenient way to understand the molecular mechanisms governing antibiotic persistence. In this opinion, we discuss our current understanding of these two phenomena, outlining how tolerance and persistence can be distinguished experimentally. We argue that this approach will help avoid controversies in the field, especially in instances where the two phenomena co-exist. Based on the premise that psychologists of religion and spirituality tend to define our objects of interest only on our own terms and not on the terms of the people we study, it is suggested that we need to be more intellectually humble in our work. The empirical study of intellectual humility is provided as a case in point. Recent research by the author and his colleagues provide evidence that research findings showing that religious people are less intellectually humble than non-religious people may be a function of how intellectual humility is being measured. When theistic humility is accounted for in the measure of intellectual humility, religious people not only demonstrate intellectual humility, but such humility predicts well-being. Existing conceptualizations and operationalizations of intellectual humility (IH) have not taken into account the way IH is experienced and expressed by theists. Based on a conceptualization of Christian theistic intellectual humility, we developed a measure of the construct, following established measure development steps of developing and reviewing scale items, exploring and confirming the factor structure, and establishing construct validity. We then tested the usefulness of the resulting scale by showing that theistic intellectual humility moderates four of the five tested relationships between religious commitment and well-being. Background: Global health security (GHS) and universal health coverage (UHC) are key global health agendas which aspire for a healthier and safer world. However, there are tensions between GHS and UHC strategy and implementation. The objective of this study was to assess the relationship between GHS and UHC using two recent quantitative indices. Methods: We conducted a macro-analysis to determine the presence of relationship between GHS index (GHSI) and UHC index (UHCI). We calculated Pearson's correlation coefficient and the coefficient of determination. Analyses were performed using IBM SPSS Statistics Version 25 with a 95% level of confidence. Findings: There is a moderate and significant relationship between GHSI and UHCI (r = 0.662, p<0.001) and individual indices of UHCI (maternal and child health and infectious diseases: r = 0.623 (p<0.001) and 0.594 (p<0.001), respectively). However, there is no relationship between GHSI and the non-communicable diseases (NCDs) index (r = 0.063, p>0.05). The risk of GHS threats a significant and negative correlation with the capacity for GHS (r = -0.604, p<0.001) and the capacity for UHC (r = -0.792, p<0.001). Conclusion: The aspiration for GHS will not be realized without UHC; hence, the tension between these two global health agendas should be transformed into a synergistic solution. We argue that strengthening the health systems, in tandem with the principles of primary health care, and implementing a "One Health" approach will progressively enable countries to achieve both UHC and GHS towards a healthier and safer world that everyone aspires to live in. Many global health institutions, including the World Health Organization, consider primary health care as the path towards achieving universal health coverage (UHC). However, there remain concerns about the feasibility and effectiveness of this approach in lowresource countries. Ethiopia has been implementing the primary health-care approach since the mid-1970s, with primary health care at the core of the health system since 1993. Nevertheless, comprehensive and systemic evidence on the practice and role of primary health care towards UHC is lacking in Ethiopia. We made a document review of publicly available qualitative and quantitative data. Using the framework of the Primary Health Care Performance Initiative we describe and analyse the practice of primary health care and identify successes and challenges. Implementation of the primary health-care approach in Ethiopia has been possible through policies, strategies and programmes that are aligned with country priorities. There has been a diagonal approach to disease control programmes along with health-systems strengthening, community empowerment and multisectoral action. These strategies have enabled the country to increase health services coverage and improve the population's health status. However, key challenges remain to be addressed, including inadequate coverage of services, inequity of access, slow health-systems transition to provide services for noncommunicable diseases, inadequate quality of care, and high out-of-pocket expenditure. To resolve gaps in the health system and beyond, the country needs to improve its domestic financing for health and target disadvantaged locations and populations through a precision public health approach. Satellite nowcasting potentially provides a vital opportunity to mitigate against the risks of severe weather in tropical Africa, where population growth and climate change are exposing an ever growing number of people to weather hazards. Numerical weather prediction demonstrates limited skill for much of Africa and weather radars are rare. However, geostationary satellites provide excellent spatial and temporal coverage of the often long-lasting convective storms that deliver heavy rain, lightning and strong winds, presenting a valuable opportunity for satellite nowcasting. Here, we evaluate the skill of satellite nowcasting products for tropical Africa: these products are routinely generated, but to our best knowledge never routinely used in tropical Africa before the Global Challenges Research Fund African SWIFT (Science for Weather Information and Forecasting Techniques) project. Focusing in particular on convective rainfall rate (CRR) and rapidly developing thunderstorm convection warning (RDT-CW) products, we demonstrate that both are useful nowcasting tools. The CRR product produces very different rainfall climatologies for day and night in tropical Africa. This is associated with greater skill of the product during daytime, particularly for heavier rain rates. The RDTCW product is able to identify around 60% of heavy (>5 mm(hr−1) rainfall events with the fraction detected increasing with increasing rainfall rate. For both products, extrapolation forwards in time (up to 90 and 60 min, respectively) maintains useful skill in tropical Africa, motivating work to develop longer lead-time nowcasts. Previous research suggests that people who feel forgiven by God may experience lower levels of death anxiety. The purpose of the current study is to contribute to this work by assessing whether the relationship between forgiveness by God and death anxiety varies according to how people view God. Three images of God are assessed: a pantheistic view of God, a theistic view of God, and a deistic view of God. Data from nationwide survey that was conducted in 2014 (N = 2,650) suggest that the relationship between forgiveness by God and death anxiety is strongest among people with a theistic view of God, significantly weaker among people with a pantheistic view of God, and not significant among individuals with a deistic view of God. Background: The third Sustainable Development Goal (SDG - 3) aims to ensure healthy lives and promote well-being for all at all ages. SDG-3 has a specific target on universal health coverage (UHC), which emphasizes the importance of all people and communities having access to quality health services without risking financial hardship. The objective of this study is to review progress towards UHC using antiretroviral treatment (ART) as a case study.Methods: We used a mixed-methods design including qualitative and quantitative approaches. We reviewed and synthesised the evidence on the evolution of the WHO HIV treatment guidelines between 2002 and 2019. We calculated ART coverage over time by gender, age group, and location. We also estimated ART coverage differences and ratios.Findings: ART guidelines have evolved from "treating the sickest" to "treating all". ART coverage increased globally from under 7% in 2005 to 62% in 2018. There have been successes in increasing ART coverage in all populations and locations. However, progress varies by population and location in many regions. There is inequity in ART coverage: women (68%) versus men (55%), and adults (62%) versus children (54%). This inequity has widened over time, and with expanded ART eligibility criteria. On the other hand, data from at least one high-burden country (Ethiopia) shows that inequity among regions has narrowed over time due to the improvements in the primary health care systems and implementation of the public health approach in the country.Conclusion: ART coverage has increased at global, regional and national levels to all population groups. However, the gains have not been equitable among locations and populations. Policies towards universality may widen the inequity in resource-limited settings unless countries take precautions and "put the last first". We argue that primary health care and public health approaches, with multi-sectoral actions and community engagement, are vital to minimize inequity, achieve UHC and leave no one behind. This article advocates for the inclusion of theistic beliefs in the study of humility and intellectual humility and recommends the construct of worldview as a promising resource for this endeavor. The promise of this approach is tested by exploring the contrasting worldviews of three religious traditions: Buddhism, Christianity, and atheism. In chapter 5 of her text, Claire White discusses what the CSR has discovered regarding how people develop beliefs about the nature of the world and how it works. Cognitive science recognizes several mechanisms that influence beliefs, both religious and nonreligious. We review some of the literature on the cognitive origin of beliefs and apply the principles identified by cognitive science to CSR beliefs about religion. What do CSR researchers believe about religion and religious experiences, and what do they believe about the CSR itself? Variables found by the CSR to influence religious beliefs are the same factors that influence all beliefs, including beliefs CSR researchers hold about the research process in which they are engaged. We propose Watson's Ideological Surround Model (ISM) as a way of handling perceived incompatibilities between beliefs about religion and religious experience held by scientific researchers and those held by religious individuals. Background: Stress is a common feature of life and has routinely been linked with negative health outcomes. However, meaning has been identified as a possible buffer against stress.Purpose: The purpose of the current study was to examine whether the relationship between stress and health was mediated by meaning in life.Methods: Drawing from Wave 1 of the Landmark Spirituality and Health Study, a nationally representative sample of adults, participants (N = 1871) reported their level of stress in the past 12 months, current meaning in life, health (measured as minor symptoms, major conditions, and overall health), and provided a blood sample for biomarker of immune system functioning (i.e., presence of Epstein-Barr virus antibodies).Results: Results revealed an indirect effects model in which stress was inversely associated with meaning. Higher meaning was related to better self-reported health (across minor, major, and overall health measures), which, in turn, was associated with better immune system functioning.Conclusions: These findings suggest that part of the negative effect of stress on health is accounted for by reduced meaning. Park's meaning-making model suggests that events such as the loss of a loved one may cause distress because people's appraised meaning of the difficult event may challenge their global orienting systems. Meaning-making alleviates distress by reducing this discrepancy. Research has shown the important role that religions often play in the meaning-making process. However, this body of research has largely been limited by a reluctance to address the religious content of meaning-making processes and outcomes. Here we advocate for inclusion of religious beliefs in the study of meaning-making in suffering, and recommend the construct of worldview as a promising resource for this endeavour. Finally, we illustrate the promise of this approach by exploring the contrasting worldviews of three religious traditions: Buddhism, Christianity, and atheism. There is growing evidence that a sense of meaning in life may emerge, in part, from the social relationships that people maintain. But it is not clear how the relationship between social ties and a sense of meaning might arise. The purpose of this study is to see if meaning in life is associated with three socially focused virtues: compassion, forgiveness of others, and providing social support to others. In the process, an effort is made to see if these social virtues arise from social relationships in religious institutions. Two main findings emerge from a recent nationwide survey. First, people who are more compassionate, more forgiving, and who help others more often have a stronger sense of meaning in life. Second, individuals who receive more spiritual support from fellow church members are more likely to adopt these social virtues. The theoretical basis of these relationships is discussed. Many bacterial infections are hard to treat and tend to relapse, possibly due to the presence of antibiotic-tolerant persisters. In vitro, persister cells appear to be dormant. After uptake of Salmonella species by macrophages, nongrowing persisters also occur, but their physiological state is poorly understood. In this work, we show that Salmonella persisters arising during macrophage infection maintain a metabolically active state. Persisters reprogram macrophages by means of effectors secreted by the Salmonella pathogenicity island 2 type 3 secretion system. These effectors dampened proinflammatory innate immune responses and induced anti-inflammatory macrophage polarization. Such reprogramming allowed nongrowing Salmonella cells to survive for extended periods in their host. Persisters undermining host immune defenses might confer an advantage to the pathogen during relapse once antibiotic pressure is relieved. [ABSTRACT FROM AUTHOR]

The contribution of cloud to the radiation budget of southern West Africa (SWA) is poorly understood and yet it is important for understanding regional monsoon evolution and for evaluating and improving climate models, which have large biases in this region. Radiative transfer calculations applied to atmospheric profiles obtained from the CERES–CloudSat–CALIPSO–MODIS (CCCM) dataset are used to investigate the effects of 12 different cloud types (defined by their vertical structure) on the regional energy budget of SWA (5°–10°N, 8°W–8°E) during June–September. We show that the large regional mean cloud radiative effect in SWA is due to nonnegligible contributions from many different cloud types; eight cloud types have a cloud fraction larger than 5% and contribute at least 5% of the regional mean shortwave cloud radiative effect at the top of the atmosphere. Low clouds, which are poorly observed by passive satellite measurements, were found to cause net radiative cooling of the atmosphere, which reduces the heating from other cloud types by approximately 10%. The sensitivity of the radiation budget to underestimating low-cloud cover is also investigated. The radiative effect of missing low cloud is found to be up to approximately −25 W m−2 for upwelling shortwave irradiance at the top of the atmosphere and 35 W m−2 for downwelling shortwave irradiance at the surface. Background: Two loop diuretics, torsemide and frusemide, can affect the urinary system and consequently the cardiordiovascular haemodynamics in different ways. Objectives: This study compared a number of echocardiographic parameters and systemic arterial blood pressure (ABP) changes following administration of furosemide or torsemide. Methods: Five shelter dogs underwent transthoracic two‐dimensional M‐mode echocardiography to obtain the following measurements: left ventricular internal dimension at end‐systole (LVIDs), left ventricular internal dimension at end‐diastole (LVIDd), fractional shortening (FS), heart rate (HR) and the distance between the mitral valve socket and the ventricle wall (septal to E Point, SEP). Arterial blood pressure was measured using the oscillometric method. Measurements recorded before treatment (baseline data) were compared to those after the dogs received furosemide (5 mg/kg) or torsemide (0.5 mg/kg). Results: Torsemide significantly reduced blood pressure 1 h after administration, but this was not seen with furosemide. Fractional shortening, LVIDd and SEP decreased following both treatments, but there were no significant differences between the treatment groups. Torsemide increased heart rate above that seen in the furosemide groups. Conclusions: The results of this study indicate that 1 h after administration, torsemide increases heart rate and decreases blood pressure when compared to furosemide, but both drugs have similar effects on measured cardiovascular indices. At September’s 2017 United Nations General Assembly, a state-of-the-art HIV medicine was announced to be made available at just $75 per person per year. There have been a number of strategies that the global AIDS community and countries have utilized to reduce prices and make antiretrovirals (ARVs) accessible for people living with HIV/AIDS. There appears to be an opportunity for the treatment of hepatitis C virus infection using direct-acting antivirals (DAAs) to benefit from the often painful and laboured history of driving down the prices of ARVs. In general, the success of lowering prices for ARVs has stemmed from the politics needed to initially support generic entry into the on-patent market. The use of flexibilities present in the World Trade Organization’s Agreement on Trade-Related Aspects of Intellectual Property Rights (TRIPS) have been used to overcome patent barriers, with the use of compulsory licenses and/or the threat of their use as instruments for strengthening the bargaining power in price negotiations. These strategies have been combined with new financing mechanisms that have promoted more effective procurement and price negotiations. Partnership among the different stakeholders has also been critical in this regard. Countries have also invested in their health systems and implemented several strategies to reduce stigma and discrimination to increase access to and improve utilization of ARVs. This article suggests that any future international initiatives to increase access to DAAs can learn from these lessons surrounding price reduction, improved financing, advocacy, as well as health systems strengthening and stigma reduction. The Marquis de Fénelon’s internationally popular didactic narrative, Les aventures de Télémaque, went through a remarkable number of metamorphoses in the Nahḍah, the Arab world’s cultural revival movement of the long nineteenth century. This article examines two early manuscript translations by Syrian Christian writers in the 1810s, the rhymed prose version by Rifāʿah Rāfiʿ al-Ṭahṭāwī in the 1860s; its rewriting by Shāhīn ʿAṭiyyah in 1885; and Saʿdallāh al-Bustānī’s musical drama of 1869, the basis for performances later in the century by the famous actor Salāmah Ḥijāzī. Placing Télémaque’s Arabic trajectory within its global vogue in the Enlightenment suggests ways of reading the Nahḍah between theories of world literature and ‘transnational mass-texts’, and more specific local histories of translation and literary adaptation. The ambiguity of Télémaque, its hybrid and transitional form, was important to its success in milieux facing analogous kinds of hybridity and transition—among them those of the Arab Nahḍah. Submarine geomorphology is one of the main tools for understanding past fluctuations of tidewater glaciers. In this study we investigate the glacial history of Mohnbukta, on the east coast of Spitsbergen, Svalbard, by combining multibeam‐bathymetric data, marine sediment cores and remote sensing data. Presently, three tidewater glaciers, Heuglinbreen, Königsbergbreen and Hayesbreen calve into Mohnbukta. Hayesbreen surged at the end of the Little Ice Age, between 1901 and 1910. The submarine landform assemblage in Mohnbukta contains two large transverse ridges, interpreted as terminal moraines, with debrisflow lobes on their distal slopes and sets of well‐preserved geometric networks of ridges, interpreted as crevasse‐squeeze ridges inshore of the moraines. The arrangement of crevasse‐squeeze ridges suggests that both landform sets were produced during surge‐type advances. The terminus position of the 1901–1910 Hayesbreen surge correlates with the inner (R.2) terminal moraine ridge suggesting that the R.1 ridge formed prior to 1901. Marine sediment cores display 14C ages between 5700–7700 cal. a BP derived from benthic foraminifera, from a clast‐rich mud unit. This unit represents pre‐surge unconsolidated Holocene sediments pushed in front of the glacier terminus and mixed up during the 1901 surge. An absence of retreat moraines in the deeper part of the inner basin and the observation of tabular icebergs calving off the glacier front during retreat suggest that the front of Hayesbreen was close to flotation, at least in the deeper parts of the basin. As the MOH15‐01 core does not penetrate into a subglacial till and the foraminifera in the samples were well preserved, the R.1 ridge is suggested to have formed prior to the deposition of the foraminifera. Based on these data we propose that a surge‐type advance occurred in Mohnbukta in the early Holocene, prior to 7700 cal. a BP, which in turn indicates that glaciers can switch to and from surge mode. The purpose of this study is to see if a pantheistic view of God (i.e., God is all around us in nature), a theistic view of God (i.e., God is the creator of the universe who protects us), and a deistic view of God (i.e., God created the universe and then left people to fend for themselves) moderate the relationship between chronic financial strain and three measures of health (self-rated health, the number of chronic conditions, and functional disability). The data come from a recent nationwide survey of adults of all ages (N = 2,768). The findings suggest that neither pantheistic nor theistic views of God offset the effects of financial strain on any of the health outcomes. In contrast, the data further reveal that the relationship between ongoing economic difficulties and all three health outcomes is more pronounced for study participants who have a deistic view of God. Background: The recent introduction of Direct Acting Antivirals (DAAs) for treating Hepatitis C Virus (HCV) can significantly assist in the world reaching the international target of elimination by 2030. Yet, the challenge facing many individuals and countries today lies with their ability to access these treatments due to their relatively high prices. Gilead Sciences applies differential pricing and licensing strategies arguing that this provides fairer and more equitable access to these life-saving medicines. This paper analyses the implications of Gilead's tiered pricing and voluntary licencing strategy for access to the DAAs.Methods: We examined seven countries in Africa (Egypt, Ethiopia, Nigeria, Democratic Republic of Congo, Cameroon, Rwanda and South Africa) to assess their financial capacity to provide DAAs for the treatment of HCV under present voluntary licensing and tiered-pricing arrangements. These countries have been selected to explore the experience of countries with a range of different burdens of HCV and shared eligibility for supply by licensed generic producers or from discounted Gilead prices.Results: The cost of 12-weeks of generic DAA varies from $684 per patient treated in Egypt to $750 per patient treated in other countries. These countries can also procure the same DAA for 12-weeks of treatment from the originator, Gilead, at a cost of $1200 per patient. The current prices of DAAs (both from generic and originator manufacturers) are much more than the median annual income per capita and the annual health budget of most of these countries. If governments alone were to bear the costs of universal treatment coverage, then the required additional health expenditure from present rates would range from a 4% increase in South Africa to a staggering 403% in Cameroon.Conclusion: The current arrangements for increasing access to DAAs, towards elimination of HCV, are facing challenges that would require increases in expenditure that are either too burdensome to governments or potentially so to individuals and families. Countries need to implement the flexibilities in the Doha Declaration on Trade Related Intellectual Property Rights agreement, including compulsory licensing and patent opposition. The first cationic Fe silyldiazenido complexes, [Fe(PP)2(NN–SiMe3)]+[BArF4]− (PP = dmpe/depe), have been synthesised and thoroughly characterised. The catalytic fixation of H2 by molecular Fe compounds is a rapidly developing field, yet thus far few complexes can effect this transformation, and none are selective for 2H4 production. Herein we report that the simple Fe(0) complex Fe(Et2PCH2CH2PEt2)2(N2) (1) is an efficient catalyst for the selective conversion of N2 (>25 molecules N2 fixed) into N2H4, attendant with the production of ca. one molecule of NH3. Notably, the reductant (CoCp*2) and acid (Ph2NH2OTf) used are considerably weaker than conventional chemical H+ and e- sources used in previous demonstrations of N2 turnover by synthetic Fe compounds. These results show that the direct catalytic conversion of N2 to the hydrazine oxidation state on molecular Fe complexes is viable and that the mechanism of NH3 formation by such systems may proceed via Fe-N2H4 intermediates. The impact of two different coupled cirrus microphysics-radiation parameterizations on the zonally averaged temperature and humidity biases in the tropical tropopause layer (TTL) of a Met Office climate model configuration is assessed. One parameterization is based on a linear coupling between a model prognostic variable, the ice mass mixing ratio q i, and the integral optical properties. The second is based on the integral optical properties being parameterized as functions of q i and temperature, T c, where the mass coefficients (i.e., scattering and extinction) are parameterized as nonlinear functions of the ratio between q i and T c. The cirrus microphysics parameterization is based on a moment estimation parameterization of the particle size distribution (PSD), which relates the mass moment (i.e., second moment if mass is proportional to size raised to the power of 2) of the PSD to all other PSD moments through the magnitude of the second moment and T c. This same microphysics PSD parameterization is applied to calculate the integral optical properties used in both radiation parameterizations and, thus, ensures PSD and mass consistency between the cirrus microphysics and radiation schemes. In this paper, the temperature-non-dependent and temperature-dependent parameterizations are shown to increase and decrease the zonally averaged temperature biases in the TTL by about 1 K, respectively. The temperature-dependent radiation parameterization is further demonstrated to have a positive impact on the specific humidity biases in the TTL, as well as decreasing the shortwave and longwave biases in the cloudy radiative effect. Background: Cambodia developed its public health system along the principles of the district model and geared its services towards managing communicable diseases and maternal and child health issues. In line with other countries in the region, non-communicable diseases have emerged as a leading cause of adult mortality. We assessed the current capacity of the Cambodian district health system to manage hypertension and diabetes, with a focus on access to medicine for these chronic conditions.Methods: A case study whereby in three purposely selected districts in an equal number of provinces a total of 74 informants were interviewed: 27 health care providers and administrators, 30 community representatives and 17 managers of specific non-communicable diseases interventions and social health protection schemes. Questions related to the World Health Organization's health system building blocks. Data analysis involved coding, indexing, charting and mapping the data. Following these exercises all information was analysed by kind of respondent and their respective answer to the question concerned. Responses by respondents of three groups of interviewees were compared when appropriate. At 14 health centres and 3 district hospitals the availability of key medicines for hypertension and diabetes in accordance with the National Essential Drug List was assessed. This was also done for essential tools and equipment to diagnose these two conditions.Results: Although there was agreement amongst nearly all interviewees that non-communicable diseases were prevalent, the district health system, including all health systems building blocks and the referral system, was inadequately developed to effectively deal with these conditions. Medicines supply was erratic and the quantity provided allowed for few patients to be treated, for a short period only, mainly at secondary or tertiary level.Conclusions: Because of the public health, social and economic importance of non-communicable diseases, a rapid response is required. Given the current Cambodian situation, such response may initially be a diagonal approach, with non-communicable diseases services integrated in the National HIV/AIDS Programme. This analysis was designed to (1) examine the impact of heparin-induced thrombocytopenia (HIT) on contemporary cardiac surgical practice and (2) describe the results of a protocol designed for early identification of the presence of the immune mechanisms involved. Consecutive patients who underwent cardiac surgery were screened postoperatively for thrombocytopenia. Patients with thrombocytopenia were tested for antiplatelet factor 4 (PF4)/heparin antibodies by ELISA and clinical evidence of thrombosis sought. Demographics, co-morbidities, operative details, and outcomes were abstracted from the departmental registry. Of 14,415 consecutive patients undergoing cardiac surgery, 1,849 patients (13%) had thrombocytopenia. Of them, 277 patients (15%) had PF4/heparin antibodies and 76 patients (4%) had both antibodies and clinical thrombosis. Antibodies were more frequent: (1) in women (p = 0.01), (2) in patients with an increased body mass index (p <0.01), and (3) in patients with clinical heart failure before surgery (p <0.01). Thirty-day mortality was greatest among the 76 patients with the triad of thrombocytopenia, antibodies, and clinical thrombosis (30%). Of the 1,849 patients with thrombocytopenia, the presence of PF4/heparin antibodies was an independent predictor of 30-day mortality (odds ratio 2.09, 95% CI 1.46 to 2.49; p <0.001). HIT remains an infrequent but very serious complication of heparin therapy in contemporary cardiac surgical practice. Summary The integrity of chromatin, which provides a dynamic template for all DNA-related processes in eukaryotes, is maintained through replication-dependent and -independent assembly pathways. To address the role of histone deposition in the absence of DNA replication, we deleted the H3.3 chaperone Hira in developing mouse oocytes. We show that chromatin of non-replicative developing oocytes is dynamic and that lack of continuous H3.3/H4 deposition alters chromatin structure, resulting in increased DNase I sensitivity, the accumulation of DNA damage, and a severe fertility phenotype. On the molecular level, abnormal chromatin structure leads to a dramatic decrease in the dynamic range of gene expression, the appearance of spurious transcripts, and inefficient de novo DNA methylation. Our study thus unequivocally shows the importance of continuous histone replacement and chromatin homeostasis for transcriptional regulation and normal developmental progression in a non-replicative system in vivo. Among dog breeds, the Dachshund has the highest lifetime incidence of intervertebral disc disease (IVDD). Intervertebral disc (IVD) calcification is an indicator of severe degeneration that predisposes to disc herniation. IVDD is heritable in Dachshunds, and in some countries, breeding candidates are screened to reduce IVDD occurrence by selecting dogs according to their score of radiographically detectable intervertebral disc calcification (RDIDC) and excluding dogs with ≥5 RDIDCs from breeding. This study evaluated the precision of scoring spinal radiographs for IVD calcification and subsequent classification of Dachshund dogs for breeding based on their RDIDC score. Digital radiographs of the spine were obtained in 19 clinically healthy, young adult Dachshunds, and scored for RDIDC independently by five scorers with varying levels of experience, three times each. Within scorer (repeatability) and between scorer (reproducibility) variability was estimated both at the individual IVD level and at the whole dog level for breeding classification purposes. At the IVD level, some degree of scorer effect was supported by the pairwise repeatability (92.3%; 95% CI: 88.8–94.7%) being marginally higher than the reproducibility (89.2%; 95% CI: 85.7–91.8%). Scorer-specific patterns confirmed the presence of scorer subjectivity. Repeatability significantly increased with scorer experience but the reproducibility did not. RDIDC scoring repeatability and reproducibility substantially decreased at the cervicothoracic spine region, likely due to anatomical superimpositions. At the dog level, a breeding classification could be repeated by the same scorer for 83.6% (95% CI: 73.8–90.2%) of the dogs, and was reproduced between two scorers for 80.2% (95% CI: 66.6–89.1%) of the dogs. The repeatability of breeding classification also seemed to increase with scorer experience but not the reproducibility. Overall, RDIDC scoring revealed some degree of inconsistency explained by scorer subjectivity and inexperience, and anatomical superimpositions. Background: The Millennium Development Goals expire at the end of 2015 and global negotiations are underway to finalise the post-2015 Sustainable Development Goals. Much activism has occurred encouraging a post-2015 health and development goal embedded in the highest attainable standard of health ('right to health'). Despite this, the right to health was absent in three key post-2015 intergovernmental Sustainable Development Goal proposals in 2014, one of which was reinforced by the United Nations General Assembly in September 2014 as the guiding document for ongoing interstate negotiations. This article examines why it appears the right to health, so far, is not gaining direct expression in post-2015 discussion. Methods: This qualitative research is part of a broader study using thematic and discourse analysis examining the high-level policy debate on health goals in the discourse of the formulation of the post-2015 Sustainable Development Goals. Key-informant interviews were conducted in two interview rounds in 2013 and 2014, with participants from multilateral and other organisations (government, academia, civil society and philanthropy) responsible for health in the post-2015 development agenda (or the post-2015 development agenda more broadly). This study synthesises data from both interview rounds on Health and Human Rights in post-2015 Sustainable Development Goal negotiations. Results: Six reasons why the right to health may not have gained effective traction in the unfolding post-2015 Member State negotiations were found. The first three reasons relate to broader issues surrounding human rights' (including sexual and reproductive health and rights) positioning within international relations discourse, and the second three relate to the challenges of transforming the human right to health into a practically applied post-2015 health goal. Conclusions: This paper reports the views of participants, many of who sit at the interface of United Nations and Member State negotiations, on the right to health's location (and projected trajectory) at two temporal junctions in evolving post-2015 negotiations. The interviews provide insight into high-level hesitancy that the right to health be expressly incorporated in the final post-2015 health and development goal, as well as documents participants' doubt that rights language will explicitly frame the broader Sustainable Development Goals, their targets and indicators. The perception that one is a recipient of divine grace is a salient spiritual experience and is one that lies at the heart of Christianity. This qualitative study sought to address a gap in the psychological literature regarding how the perceived experience of divine grace enacts internal change. For the sake of this study, grace was defined as God's undeserved gifts to believers. Thirty adult, self-identified evangelical Christians were interviewed to explore the perceived relationship between God's grace and the cultivation of humility. Using grounded theory data analysis, themes emerged to reveal that participants expressed a dynamic relationship between receiving God's grace and three dimensions of humility related to (a) a grounded view of oneself, (b) an orientation toward others, and (c) an overall dependence on God. Background Grading of erythema in clinical practice is a subjective assessment that cannot be confirmed using a definitive test; nevertheless, erythema scores are typically measured in clinical trials assessing the response to treatment interventions. Most commonly, ordinal scales are used for this purpose, but the optimal number of categories in such scales has not been determined. Hypothesis/Objectives This study aimed to compare the reliability and agreement of a four-point and a six-point ordinal scale for the assessment of erythema in digital images of canine skin. Methods Fifteen digital images showing varying degrees of erythema were assessed by specialist dermatologists and laypeople, using either the four-point or the six-point scale. Reliability between the raters was assessed using intraclass correlation coefficients and Cronbach's α. Agreement was assessed using the variation ratio (the percentage of respondents who chose the mode, the most common answer). Intraobserver variability was assessed by comparing the results of two grading sessions, at least 6 weeks apart. Results Both scales demonstrated high reliability, with intraclass correlation coefficient values and Cronbach's α above 0.99. However, the four-point scale demonstrated significantly superior agreement, with variation ratios for the four-point scale averaging 74.8%, compared with 56.2% for the six-point scale. Intraobserver consistency for the four-point scale was very high. Conclusions and clinical importance Although both scales demonstrated high reliability, the four-point scale was superior in terms of agreement. The electron-deficient and sterically bulky trialkylborane derivative tris[bis(pentafluorophenyl)methyl]borane [1, B(CH(C6F5)2)3], has been synthesised and comprehensively characterised; detailed 1H and 19F NMR studies reveal two dynamic bond rotational processes in the solution phase. Despite conventional probes (Gutmann–Beckett and Childs methods) implying that the compound has a very limited Lewis acidity, it was used to generate frustrated Lewis pairs capable of heterolytically activating H2 in ethereal solutions, which suggests that the hydridophilicity of 1 is comparable to the potent Lewis acid B(C6F5)3. Cell identity is a reflection of a cell type-specific gene expression profile, and consequently, cell type-specific transcription factor networks are considered to be at the heart of a given cellular phenotype. Although generally stable, cell identity can be reprogrammed in vitro by forced changes to the transcriptional network, the most dramatic example of which was shown by the induction of pluripotency in somatic cells by the ectopic expression of defined transcription factors alone. Although changes to cell fate can be achieved in this way, the efficiency of such conversion remains very low, in large part due to specific chromatin signatures constituting an epigenetic barrier to the transcription factor-mediated reprogramming processes. Here we discuss the two-way relationship between transcription factor binding and chromatin structure during cell fate reprogramming. We additionally explore the potential roles and mechanisms by which histone variants, chromatin remodelling enzymes, and histone and DNA modifications contribute to the stability of cell identity and/or provide a permissive environment for cell fate change during cellular reprogramming. This essay examines the translations into Arabic of The Pilgrim's Progress and Robinson Crusoe, two of the first works of modern prose fiction to be translated into Arabic in the nineteenth century. Both are didactic and allegorical narratives that belong to the prehistory of the European realist novel; they were originally written in the late seventeenth to early eighteenth centuries, and by the early nineteenth century had become among the most popular fictional works in Europe, both in their original languages and in translation. They were translated into Arabic at various stages of the nineteenth-century revival (nahḍa). The translations of each work are dealt with separately, but demonstrate certain shared issues and problems of translation in this period. These include the negotiation of different levels of language within Arabic, as suitable local equivalents for the translated work are found; the vital role of individual effort, and the placing of this within a wider educational and patriotic project; the awareness of European and local structures of power; and most importantly of all, perhaps, a shared didactic impulse. The two translations of The Pilgrim's Progress provide evidence of a shift of approach towards translation within the missionary project. The first translation (1834) belongs largely to an Evangelical mode of universal translatability, which tends to ignore distinctions between languages and between levels and registers within a language, in an attempt to proselytize as many people as possible. The second translation was made only ten years later for the American missionaries by the Protestant convert Buṭrus al-Bustānī. It belongs to a more sensitive mode of free translation into a literary idiom influenced by adab norms, and aimed at an educated readership. The two translations also — despite evidence of their appeal to at least fairly wide Christian audiences — seem to have been especially relevant to Syrian Protestant converts, for whom the radically solitary experience of Bunyan's protagonist Christian corresponded to their own experiences of personal conversion. Robinson Crusoe was also translated twice — by missionaries in an abridged version in 1835, and in a full version, outside the missionary framework, in 1861. This second translation was made by Buṭrus al-Bustānī, after his period of close collaboration with the missionaries had ended and he had moved to more ecumenical educational and patriotic projects. His translation demonstrates both continuity with and breaks away from the earlier missionary translations of both The Pilgrim's Progress and Robinson Crusoe. His version of Robinson C rusoe is still presented as a didactic and exemplary narrative, more than a realist novel; an emphasis on the individual struggle and suffering of the protagonist is preserved, as are markers of the work's Christian and Puritan provenance. Attention-deficit hyperactivity disorder (ADHD) is a heterogeneous neurobehavioural syndrome, complex in aetiology and presentation. It is characterised by symptoms of inat tention, hyperactivity and impulsivity of varying combinations and degrees of severity. This article seeks to elucidate the different components of the assessment and diagnosis of ADHD in comparison with various differential diagnoses that must also be considered. Despite its complexities, ADHD can be effectively managed using a comprehensive approach advocated by the National Institute for Health and Care Excellence (NICE). The article discusses the combination of medication, psychoeducation and parental handling that has been shown to be effective in containing the problematic symptoms of ADHD. Empirical studies have identified significant links between religion and spirituality and health. The reasons for these associations, however, are unclear. Typically, religion and spirituality have been measured by global indices (e.g., frequency of church attendance,self-rated religiousness and spirituality) that do not specify how or why religion and spirituality affect health. The authors highlight recent advances in the delineation of religion and spirituality concepts and measures theoretically and functionally connected to health. They also point to areas for growth in religion and spirituality conceptualization and measurement. Through measures of religion and spirituality more conceptually related to physical and mental health (e.g., closeness to God, religious orientation and motivation, religious support, religious struggle), psychologists are discovering more about the distinctive contributions of religiousness and spirituality to health and well-being.
