33 CT-investigations of 23 patients with malignancies of the shoulder girdle were analyzed. In 80% of the surgically treated bone tumours of 17 patients the presence and extend of soft tissue infiltration and of osseous destruction could be delineated. The size of the 6 soft tissue tumours was better demonstrated by CT than by any other radiologic procedure. As CT is the most reliable noninvasive method to visualize the extend of a tumour of the shoulder girdle it is a valuable aid in treatment planning. Diagnostic pitfalls and results of follow-up studies are discussed.

Problem The major problem of reconstructing the anterior cruciate ligament (ACL) with the semitendinosus tendon (STT) is the low primary stability of the STT graft fixation techniques. Methods On 10 cadaver knees a surgical procedure for arthroscopic reconstruction of the ACL with the double looped STT was performed. The femoral attachment was achieved with an "U-formed" tendon wrapped around a bone-plug, which then was blocked in a single-staged femoral bone tunnel. The tibial fixation was attained using a bone-plug and an interference-screw. Anatomic studies were performed on the STT of the cadaver knees. Isometrical determinations were performed intraoperatively for precise anatomical placement of the tendon. We compared these measurements with the postoperative position of the STT-transplant. The tibial and the femoral attachment was tested separately for primary stability and stiffness. The mode of failure of the fixation methods was determined. Results The average length of the STT was 29.4 cm (SD +/- 3.6 cm, 24-33). With comparing the measured intraoperative and postoperative isometrical values we found a high correlation. The primary stability of the attaching methods in the pullout-testing amounted for the femoral attachment to 448.2 N (SD +/- 76.5 N, 322-560 N) and for the tibial attachment to 440.0 N (SD +/- 72.5 N, 316-538 N). The average stiffness of the femoral fixation was 29.23 N/mm (SD +/- 1.64 N/mm; 26.6-32.1 N/mm). The average stiffness of the tibial fixation measured 28.31 N/mm (SD +/- 1.55 N/mm, 25.1-30.4 N/mm). Conclusion The presented method provides an economical and easy to perform autologous STT-fixation technique with a high level of primary stability.

In den Jahren 1994 bis 1996 wurde an der unfallchirurgischen Klinik des Krankenhauses Nordwest, Frankfurt am Main, bei 26 Patienten (Durchschnittsalter 25,1 Jahre, 16–43 Jahre) ein autologes Operationsverfahren zur arthroskopischen Rekonstruktion des vorderen Kreuzbands (VKB) mit der gedoppelten Semitendinosussehne (STS) durchgefuhrt. Die femorale Befestigung erfolgte, indem die Sehne „U-formig“ um einen Knochenzylinder geschlungen und dieser in einem einstufigen femoralen Bohrkanal verblockt wurden. Die tibiale Fixation wurde mit einem Knochenblock und einer Interferenzschraube vorgenommen. Experimenteller Teil: An 10 Leichenknien fanden anatomische Studien zur STS statt. Wir fuhrten praoperativ, nach artifizieller VKB-Ruptur und postoperativ klinische Tests (vordere Schublade, Lachmann) and KT-1000-Messungen durch. Zur genauen anatomischen Plazierung der Sehne und bei liegendem STS-Transplantat wurden Isometriebestimmungen vorgenommen. Die Primarstabilitat der Fixationsmethoden im Ausrisexperiment lag fur die femorale Fixation bei durchschnittlich 448,2 N und fur die tibial Fixation bei 440,0 N. Klinischer Teil: Der Nachbeobachtungszeitraum betrug im Mittel 22,5 Monate (4–40). Die Auswertung erfolgte anhand des Insall-knee-Scores (Punktmittelwert [Pmtw] praoperativ: 38,1, postoperativ: 95,7), des Insall-functional-Scores (Pmtw praoperativ: 64,3, postoperativ 96,2) und des Lysholm-Scores (Pmtw praoperativ: 53,6, postoperativ: 96,2). Im IKDC-Score ergaben sich 26,9% sehr gute, 65,4% gute und 7,7% masige Ergebnisse. Die postoperativ durchgefuhrten KT-1000-Messungen zeigten im Seitenvergleich bei 92,3% der Patienten stabile Kniegelenke (a.-p. Translation < 2 mm bei 89N). 80,8% der Patienten, darunter 4 Leistungssportler, kehrten zu ihrem ursprunglichen Aktivitatsniveau zuruck. Fazit: Vorgestellt wird ein kostengunstiges, einfach durchzufuhrendes, rein autologes femorales STS-Fixationsverfahren mit hoher Primarstabilitat.

The benefit of antibiotics applied intravenously to avoid postoperative infection after intramedullary nailing has not yet been proven. Local antibiotic-releasing systems might be more effective because of the high concentration available in the compartments of interest. We report on the pharmacokinetic results of ten patients with intramedullary nailing of the tibia, respectively 15 patients undergoing the same procedure at the femur, in whom we used gentamicin-impregnated biomaterials for perioperative prophylaxis. The elution of gentamicin from a collagen sponge was complete and pharmacokinetic data suggest this drug releasing system is superior to PMMA beads in respect of the studied indication.

After operating on the injured cruciate ligaments, immediate mobilization and protection of the healing ligament are indispensible. However, no approach has yet been shown to safely provide both. Therefore, in 1984 we introduced a new technique: the repaired or reconstructed cruciate ligaments are augmented with a biodegradable material - a doubled-over PDS cord, 1.5 mm in diameter. It allows for immediate and safe mobilization. Because of its appropriate elasticity it does not cause "stress shielding." Postoperatively, the patients are mobilized on a motorized frame for 2 weeks. Subsequently, we have used a limited mobilization cast (LMC). Forty-nine out of 63 patients (78%) have now been reexamined for this follow-up study, using the score from the Hospital for Special Surgery and stress X-rays in the Lachmann, position (30 degrees flexion). The results compare favorably with a prior follow-up study of patients from our hospital without PDS augmentation and without early mobilization. The knees in the present study are more stable and have a better range of motion, particularly when the LMC was used.

Es wird uber den Einflus von Cyclophosphamid (Ctx) auf die Knochenbruchheilung des Kaninchenfemurs berichtet. Rontgenologisch zeigte sich unter Gabe von Ctx eine verminderte Kallusbildung, was sich fluoreszenzmikoskopisch in der verringerten Knochenanbaurate darstellte. Wahrend Bruchlast, Bruchenergie und Mineralsalzgehalt am nichtoperierten Knochen durch die Behandlung mit Ctx keine Anderung erfuhr, sank die Bruchlast und die Bruchenergie 4 Wochen post operationem um nahezu um die Halfte, und der Mineralsalzgehalt war signifikant herabgesetzt.

In einer vergleichenden experimentellen Untersuchung wurde die Biege- und Rotationsbelastbarkeit stabiler und instabiler pertrochanterer Frakturen nach Versorgung mit elastischen Rundnageln nach Simon-Weidner-Ender und 130°-Winkelplatte der AO gepruft. Diese biomechanisch unterschiedlichen Osteosyntheseformen wahlten wir stellvertretend fur die konkurrierenden intra- und extramedullaren Osteosyntheseverfahren.

Die Neutralisation der Biege- und Schubbelastungen am medialen Tragpfeiler bei instabilen pertrochanteren Frakturen durch die Verbund-Osteosynthese last hohe Anfangsfestigkeiten zu. Aufgrund klinischer Erfahrungen mit 44 Verbundosteosynthesen, davon bei 35 per- und subtrochanteren Femurfrakturen, zeigte sich, das es in 11 Fallen zu frakturspezifischen Komplikationen wie Implantatbruchen, Pseudarthrosen etc. kam. Die Ursache scheint in einer verzogerten Bruchheilung infolge Implantation des Knochenzementes in den Frakturbereich zu liegen. Die Analgerung autologer Spongiosa wird empfohlen. Die Indikation zur Verbund-Osteosynthese bleibt beschrankt auf instabile pertrochantere Frakturen beim alten Patienten in schlechtem Allgemeinzustand und kurzer Lebenserwartung und auf pathologische Frakturen bei Patienten mit allgemeiner Metastasierung.

Es sollte die Wirkung unphysiologischer Muskelkrafte auf wachsende Rattenknochen untersucht werden. 35 Tieren wurde je ein Femur total exstirpiert, und 40 Tiere dienten als Kontrolltiere. Die Tiere wurden entweder sofort oder nach sechs, 12 oder 24 Wochengetotet. Die makroskopischen, rontgenologischen und fluoreszenzmikioskopischen Ergebnisse stimmen uberein. Die vermehrt belasteten Tibiae zeigten gegenuber den Kontrolltibiae grosere Knochendichte, verstarkten Anbau ventral und lateral, hauptsachlich im proximalen Drittel, sowie Torsion im Querschnitt. Die vermindert belasteten Tibiae zeigten dagegen geringere Knochendichte, verstarkten Anbau medial und lateral, hauptsachlich im proximalen Drittel, sowie Torsion im Querschnitt. Die Ergebnisse werden durch die unterschiedlich wirksamen Druck- und Muskelkrafte erklart und mit den Ergebnissen anderer Autoren verglichen.

Die operative Knochenbruchbehandlung mit intra- und extraossaren Krafttragern sowie die Knochentransplantation setzen eine genaue Kenntnis der Gefasversorgung und des Verhaltens des ossaren Gefassystems nach operativen Eingriffen voraus.

Die Bruckenkallusbildung als Komplikation nach Unterarmfrakturen bedingt eine erhebliche Einschrankung der Drehbewegungen der Hand. Ursachlich ist eine Verknocherung, die ihren Ursprung am Frakturspalt hat und sich in Faserrichtung der Membrana interossea auf den gegenuberliegenden Knochen hin entwickelt. Die Membrana interossea verfugt uber eine grose Potenz zur Verknocherung. Der Grund ist in ihrer Ontogenese zu suchen. Die Therapie bereitet wegen der Rezidivneigung grose Schwierigkeiten. In der Resektion des Bruckenkallus und der gedoppelten Interposition eines breiten Kutisstreifens sehen wir eine erfolgreiche Therapiemoglichkeit. Drei Patienten sind auf diese Weise erfolgreich operiert worden.

Auf Grund toxikologisch gewonnener Befunde bei frischen Unfallverletzten wird auf die Bedeutung der Einnahme von Medikamenten auf die Verkehrstuchtigkeit hingewiesen. Die Zahl der Unfallverletzten, bei denen kurz nach dem Unfall toxikologisch Medikamente nachgewiesen wurden, die in der Lage waren, die Verkehrstuchtigkeit zu beeintrachtigen, ist mit 68% erschreckend hoch. Die Ergebnisse zeigen, das der unkontrollierten Medikamenteneinnahme bei Verkehrsteilnehmern weit grosere Beachtung geschenkt werden mus als bisher.

Es wird uber eine Methode der Methacrylat-Knocheneinbettung berichtet, mit der unentkalkte Schliffpraparate von 70–100 µ Dicke und bis zu 8 × 12 cm Grose fur die polarisations-und fluoreszensoptische Untersuchung hergestellt werden konnen. Hierfur wurden die bekannten Verfahren zur Gewinnung kleinerer Knochenpraparate modifiziert.

Vom 1. 1. 1974 bis 30. 6. 1982 wurden 91 Patienten mit 108 Frakturen an der Mittelhand und/oder den Fingern operiert. 70 Patienten (76,9%) mit 81 operierten Frakturen konnten durchschnittlich 33/4 Jahre nach der Operation nachuntersucht werden. 24,3% der Patienten wiesen neben der Frakturbegleitende Weichteilverletzungen auf; 25,7% erlitten mehrere Frakturen. Von den 81 operierten Frakturen entfielen 41 auf die Mittelhandknochen und 40 auf die Finger. Die Ergebnisse wurden an Hand eines hierfur erstellten Bewertungsschemas klassifiziert, welches folgende Parameter enthalt: Beweglichkeit, Trophik und Sensibilitat, Greiffunktionen, grobe Kraft und den Rontgenbefund. Auch die subjektiven Beschwerden gingen in die Bewertung ein. Die Gesamtergebnisse bei Mittelhandfrakturen mit 92% sehr guten oder guten Resultaten gegenuber 7% befriedigenden und bei Fingerfrakturen mit 60% sehr guten oder guten gegenuber 36% befriedigenden und 3% schlechten Ergebnissen bestatigen den grosen Wert der Osteosynthese in der Handchirurgie, zumal die operierten Patienten eine negative Selektion der mit Frakturen des Handskeletts behandelten Patienten darstellen. Die Ergebnisse wurden nach Frakturlokalisation, Schwere der Begleitverletzungen und Art der durchgefuhrten Osteosynthese aufgegliedert und ausgewertet.

Das Verfahren der Wahl in der Behandlung der offenen Frakturen ist die primare operative Versorgung: An erster Stelle steht die mechanische Reinigung, die Spulung und die Wundausschneidung; danach folgt die Stabilisierung der Fraktur durch eine gewebeschonend durchgefuhrte Osteosynthese. Nachuntersucht wurden 307 operativ stabilisierte offene Frakturen aus dem Zeitraum 1973 bis 1979 durchschnittlich ca. drei Jahre nach dem Unfall. Die Knocheninfektrate betragt bei den offenen Unterschenkelfrakturen durchschnittlich 8,5%, bei den offenen Frakturen der ubrigen Lokalisation 4,9%. Am Unterschenkel wird die Osteitisrate bei Anwendung des Fixateur externe seit 1977 bei den zweit- und drittgradig offenen Frakturen auf 5,5% gesenkt. Wegen verzogerter Frakturheilung bei alleiniger Anwendung des Fixateur externe ziehen wir bei den zweit-und drittgradig offenen Unterschenkelfrakturen die sogenannteKombinationsosteosynthese vor: Die Fraktur wird zunachst exakt reponiert und mit Zugschrauben, gelegentlich auch durch eine Zweilochplatte unter Kompression gesetzt. Danach wird zusatzlich der Fixateur externe angebracht. Auserdem empfielt es sich bei den offenen Unterschenkelfrakturen grundsatzlich, eine autologe Spongiosa-Transplantation durchzufuhren, um die haufig verzogerte Frakturheilung zu beschleunigen.

Die Prophylaxe der posttraumatischen Osteomyelitis umfast eine Vielzahl ineinandergreifender Masnahmen. Durch geeignete hygienische Anordnungen wird eine Ausbreitung pathogener Keime im Krankenhaus und eine Kontamination des Unfallverletzten reduziert. Daneben sind fruhzeitige schonende pra- und postoperative Versorgung der Weichteile, technisch einwandfreie Osteosynthese unter Beachtung biomechanischer Prinzipien sowie Wahl des geeigneten metallischen Implantates von besonderem Einflus auf die Rate infektioser Komplikationen. Die Therapie der posttraumatischen Osteomyelitis folgt einem weitgehend standardisierten Schema mit Exzision infizierten Gewebes, Sequestrotomie, ggf. Spul-Saugdrainage, biomechanisch stabiler Fixation der Fragmente und autologer Spongiosatransplantation. Wahrend die Osteomyelitisquote nach Osteosynthesen geschlossener Frakturen in unserem Krankengut 0,7% betrug, musten wir bei offenen Bruchen (mit uberwiegender Beteiligung des Unterschenkels) mit etwa 9% Knocheninfekten rechnen. Um die unbefriedigend hohe Infektquote des offenen Unterschenkelbruches zu reduzieren, verwenden wir seit etwa 1 Jahr statt der Platte haufiger den Fixateur externe mit vorlaufig gutem Ergebnis.

In einer vergleichenden Untersuchung wurde die Biegebelastbarkeit stabiler und instabiler pertrochanterer und subtrochanterer Frakturmodelle nach Osteosynthesen am Knochenpraparat mit Y-Nageln, Ender-Nageln, 130°-Winkelplatten und 95°-Kondylenplatten der AO getestet. Bei den instabilen Bruchformen wurden auch Verbundosteosynthesen mit Knochenzement gepruft. Bei der stabilen Form der pertrochanteren Fraktur ist die Ender-Nagelung mit durchschnittlich 147 kp, die Winkeplatten-Osteosynthese mit ca. 200 kp belastbar. Bei fehlender medialer Abstutzung gelingt eine hochbelastbare Osteosynthese mit den gepruften Methoden nur dann, wenn der Defekt am Adam'schen Bogen mit Knochenzement aufgefullt wird. Die mit mehr als 300 kp belastbare Kondylenplatten-Verbundosteosynthese der instabilen per- und subtrochanteren Fraktur erwies sich bei einmaliger Belastung stabiler als die anderen getesteten Verfahren. Die Versuchsergebnisse sind synoptisch in Tabelle 4 zusammengefast.

An 24 Hunden werden die Heilvorgange bei 2 cm grosen Segmentdefekten des Femurschaftes untersucht. Zur Stabilisierung werden 2 Metallplatten oder ein autologer Kortikalisspan und eine Metallplatte verwendet (jeweils n=12). Der Versuchszeitraum betragt 12 und 40 Wochen. Es werden radiologische Verlaufskontrollen sowie szintigraphische Untersuchungen und Mineralgehalt-Bestimmungen durchgefuhrt. Die AuffUllung der gesetzten Defekte mit neuem Knochen erfolgt hauptsachlich von der proximalen, in geringerem Umfang auch von der distalen Resektionsgrenze her. Die Bildung der anfanglich meist kegelformigen Regenerate geht von der angrenzenden Markhohle aus. Der stabil befestigte autologe Kortikalisspan fordert die Osteogenese im Defekt. Bei den Osteosynthesen mit Hilfe des autologen Kortikalisspanes ist das naherungsweise bestimmte Volumen der knochernen Regenerate etwa 3 fach groser als bei den Doppelplatten-Osteosynthesen. Das angelagerte Kortikalistransplantat verbindet sich innerhalb von 12 Wochen fest mit der proximalen und der distalen Femurkortikalis. Dagegen entsteht bei den Doppelplattenosteosynthesen in keinem Fall eine feste knocherne Verbindung zwischen dem proximalen und dem distalen Fragment. Die Szintigraphie mit99mTc-MDP erlaubt eine Aussage uber die Aktivitat der Heilvorgange bei segmentalen Knochendefekten.

In Erganzung zu den radiologischen Befunden (siehe Teil I) zeigt sich, das die histologischen Regenerationsvorgange bei Segmentdefekten an Rohrenknochen von der Markhohle ausgehen. Es wird ein endostales Geflechtknochen-Gerust gebildet, an der proximalen Resektionsstelle in starkerem Umfang als an der distalen. Die Knochenneubildung im Defekt ist gebunden an die Revaskularisierung durch medullare Gefase. Die starkere Osteogenese an der proximalen Resektionsstelle entspricht der starkeren Gefasneubildung in diesem Bereich. Die Vaskularisierung und Auffullung von Segmentdefekten an Rohrenknochen wird beschleunigt, wenn zur Stabilisierung anstelle einer Doppelplatten-Osteosynthese nur eine Metallplatte und ein gegenuberliegender autologer Kortikalisspan verwendet werden. Der Span verbindet sich innerhalb von 12 Wochen durch Bildung eines Geflechtknochen-Gerustes an den Anlagerungsstellen mit dem Wirtsknochen. Der stabil befestigte autologe Kortikalisspan wird von Knochen- und Weichteillager her revaskularisiert. Darauf folgen Osteolyse und gleichzeitig Osteogenese, wodurch der avitale Kortikalisspan innerhalb von ca. 40 Wochen durch neugebildeten Knochen weitgehend ersetzt wird. Der dabei entstehende Stabilitatsverlust gefahrdet die Osteosynthese nicht.

Because of the anatomical and functional peculiarities of the astragallus surgical and conservative treatment of central fractures are often unsatisfactory. A follow-up study of 40 patients with 42 central talus fractures were performed over an average of 9 years after the initial trauma. The number of poor results was extremely high; only primary surgical or conservative treatment showed good or fair results. Best results were obtained by primary surgical treatment whereas fractures operated at later stages showed worse results. Therefore fresh central astragallus fractures should be operated upon, fractures older than 1 day should be treated conservatively. If a satisfactory reduction cannot be obtained, the fracture should be operated upon in order to prevent arthrosis in the upper ankle joint.

Our results and experience with computer tomography (CT) were analyzed with respect to 41 patients with fresh bone injuries in both the cervicothoracic and the lumbar region of the vertebral column. Besides differentiated representation of the involvement of various parts of the vertebral body, the arch, transverse and spinous processes in 46 fractures, and four luxation fractures, bone fragments were found in the spinal cavity in 12 patients. Two patients displayed spinal bleeding and 3 showed a traumatic vertebral disc protrusion; 5 patients had paravertebral hematomas. Comparison of the CT-verified computer-tomographic fractures with the results of conventional X-ray diagnosis in two planes, as well as partial a.p. or lateral tomography (n = 12), revealed that in 13 patients even the detection of the fracture remained uncertain. Post-traumatic CT investigation (n = 9) showed syringomyelia in 3 patients, dense necrosis in 1 patient, and spinal channel stenoses as well as radicular irritations which could be explained by overshoot bone neogenesis at the intervertebral foramina. New technological developments in CT instruments of the most recent generation (digital radiography, sagittal and coronary image reconstruction) have made CT the most effective method in the diagnosis of spinal trauma. This necessitates a reorientation in the diagnostic procedure in patients with traumatic lesions of the spine.

In einer vergleichenden Untersuchung wurde die dynamische Belastbarkeit verschiedener extra- und intramedullarer Osteosyntheseverfahren bei stabilen und instabilen pertrochanteren Frakturen gepruft. Fur die stabilen pertrochanteren Frakturen erweisen sich intramedullare Osteosyntheseverfahren (z. B. Ender-Nagel und Trochanternagel nach Kuntscher) als biomechanisch gunstiger. Extramedullare Osteosynthesen (z. B. 130° Winkelplatte der AO) gewahrleisten ebenfalls eine hohe Festigkeit, werden jedoch unter Dauerbelastungen plastisch verformt. Bei den instabilen Bruchformen werden nur von den Verbundosteosynthesen ausreichende Festigkeitswerte unter Wechseldruckbelastung erreicht. Die Bedeutung der Wiederherstellung eines tragfahigen medialen Pfeilers fur die Biomechanik der Osteosynthese wird unterstrichen. Nach den vorliegenden Ergebnissen ist vor allem die 95° Kondylenplatten-Verbundosteosynthese als sofort belastungsstabiles Verfahren zur Versorgung instabiler pertrochanterer Frakturen beim greisen Patienten geeignet.

In part I of this paper the radiological findings in segmental defects of long bones were discussed. The histological regeneration originates from the marrow cavity. An endostal framework of bone is formed; this is more pronounced on the proximal resection place than on the distal one. The new bone formation within the defect depends on the revascularization through medullar blood vessels. The increased osteogenesis on the proximal site of resection is due to the increased formation of vessels in this area. The vascularization and the filling of segmental defects in tubular bone occurs faster, if only one metal plate and on the opposite side an autologous bone graft (cortical bone) is used in order to stabilize the bone. This method is superior to the use of a double-plate osteosynthesis. The graft unites with the underlying bone within twelve weeks by forming a trabecular network of new bone. If the autologous bone graft has become stable, it is vascularized from the bone and from the sorrounding soft tissue. Afterwards an osteolysis and at the same time an osteogenesis take place. The dead bone graft is replaced by newly formed bone within about 40 weeks. The loss of stability which occurs does not endanger the osteosynthesis.

A bilateral aplasia of the dorsal arches of the atlas with persisting tuberculum posterius has been observed by the authors. Since the patient had suffered a severe cranio-cerebral trauma, it became necessary to exclude a possible fracture by differential diagnosis. This was possible by means of CT and conventional x-ray diagnosis. According to the classification of dorsal atlas arch aplasia by von Torklus and Gehle (1975) into 6 types, the diagnosis was, in the case under report, Keller's type of aplasia of the arch of the atlas.

Huftpfannenfrakturen sind vergleichsweise seltene Verletzungen, die als Folge schwerer direkter und indirekter Gewalteinwirkung entstehen. Als Unfallart steht daher der Verkehrsunfall (89%) ganz im Vordergrund [2].

Ectopic ossifications occassionally occur after stable osteosyntheses. The literature and our own observations indicate that osseous bore-chips probably favour these ossifications, especially after intermedullary nailing. Experiments determined the weight of osseous bore-chips which occur in intermedullary nailing of femur and in plate osteosynthesis of radius and ulna. Conclusions drawn from these findings should be considered when performing osteosynthesis.

Because of the anatomical and functional peculiarities of the astragallus surgical and conservative treatment of central fractures are often unsatisfactory. A follow-up study of 40 patients with 42 central talus fractures were performed over an average of 9 years after the initial trauma. The number of poor results was extremely high; only primary surgical or conservative treatment showed good or fair results. Best results were obtained by primary surgical treatment whereas fractures operated at later stages showed worse results. Therefore fresh central astragallus fractures should be operated upon, fractures older than 1 day should be treated conservatively. If a satisfactory reduction cannot be obtained, the fracture should be operated upon in order to prevent arthrosis in the upper ankle joint.

Early operated knee band lesions (n = 61) had better end results than in those later treated (note 2.3 versus 2.6). In knee luxations (n = 8) we had an average result of 3.1. In combined knee band lesions (n = 39) the persistent instability of the anterior cruciate ligament was the main problem. In such lesions we had better results with primary substitution of the anterior ligament by a gracilis tendon graft than with suture of the torn band.

In a comparative experimental study stability of different extra- and intramedullar methods of osteosynthesis for stable and unstable pertrochanteric fractures has been examined under dynamical stress. For the treatment of stable pertrochanteric fractures in old patients the intramedullar methods of osteosynthesis (nailing by Ender of Küntscher) were shown to be biomechanically superior. Extramedullar osteosyntheses (130 degrees and 95 degrees angled plate) also guaranteed very high stability; under permanent stress however angled plates showed metal fatigue. In case of unstable pertrochanteric fractures the osteosyntheses with additional application of bone cement produced sufficient stability under permanent stress. The importance of reconstructing a stable medial support for the biomechanics of osteosynthetic materials was emphasized. According to our results especially the combination of 95 degrees angle-plates with bone cement permits immediate mobilisation of old patients in case of unstable pertrochanteric fractures.

Ninety-one patients with 108 fractures of the metacarpal and/or finger bones were treated by surgery between January 1974 and June 1982. 70 patients (76.9%) with 81 surgically treated fractures could be checked up after 3 3/4 years on an average. 24.3% out of these patients suffered from associated soft tissue leasions, 25.7% suffered from multiple fractures. 41 out of the 81 surgical interventions were performed for fractures of the metacarpus and 40 for fractures of the finger bones. The results were classified according to an evaluation scheme established for this purpose and containing the following parameters: mobility, trophism and sensitivity, gripping functions, force, and X-ray findings. Subjective troubles were evaluated, too. The great value of osteosynthesis in the surgical treatment of the hand is confirmed by the overall results with 92% of very good or good results and 7% of satisfactory results in metacarpal fractures and 60% of very good or good results, 36% of satisfactory results and 3% of poor results in fractures of the finger bones, the more as patients treated by surgery represent a negative selection among all patients treated for fractures of the bones of the hand. The results were classified and evaluated according to fracture site, severity of associated injuries, and method of osteosynthesis applied.

Um eine Vorstellung uber die Strahlenbelastung von Patient und strahlenexponiertem Personal im Rahmen der Unfallchirurgie zu erhalten, wurden Strahlenschutzmessungen am Phantom und bei Eingriffen zur Osteosynthese von Schenkelhals-und Oberschenkelfrakturen durchgefuhrt, bei denen die Durchleuchtung mit Bildverstarker-Fernseheinrichtungen nicht zu entbehren ist. Die Meswerte intra operationem bei 24 Patienten (Schenkelhalsnagelung mit Winkelplatte und Ender-Nagel, Oberschenkelmarknagelung nach Kuntscher einschlieslich Verriegelungs-Nagelung) zeigen, das es bei sachgemaser Operationsvorbereitung, Verwendung moderner Bildverstarkereinrichtungen mit Streustrahlenraster und Belichtungsautomatik und guter Durchleuchtungsdisziplin moglich ist, die Strahlenexposition von Patient und Personal unter der Grenze zu halten, wie sie im Rahmen der rontgenologischen Magen-Darm-Diagnostik ublich ist. Unter diesen Voraussetzungen konnen etwa 8 Operationen pro Woche durchgefuhrt werden, ohne den maximal zulassigen Wert fur Teilkorperdosis des Chirurgen zu uberschreiten. Das tatsachlich durchgefuhrte Operationsprogramm hatte noch erweitert werden konnen. Der Kontrollbereich im Operationssaal kann in 2 m Entfernung vom Patienten festgelegt werden. Die zu beachtenden Strahlenschutzmasnahmen werden erlautert.

In unstable pertrochanteric fractures of the femur a high bending strain and much load lies on the medial part of the bone. The connecting osteosynthesis takes off this load and immediately guarantees a high stability. Clinical experiences with 44 cases of connecting osteosynthesis - among them 35 per-and subtrochanteric fractures of the femur-show, that in 11 cases complications due to the fractures occurred (breaking of the implant, pseudarthrosis etc.) These complications are probably due to a delayed healing caused by bone cement penetrating into the area of the fracture. It is recommended to use an autologous cancellous bone graft. Indications for the compound osteosynthesis are only unstable pertrochanteric fractures in old patients in poor general condition and a short life-expectancy, and fractures in patients with diffuse metastases.

The authors investigated in 25 patients with luxations of the patella if an increased incidence of defective torsions of the leg skeleton could be demonstrated. The torsion conditions of the lower extremity can be easily shown by computed tomography. This was confirmed when 32 femur preparation and 95 sound extremities were examined. The comparison of measuring results does not show any difference between patients and control group regarding the torsion of tibia and femur and the position of the tuberosity of the tibia. Patients with luxations of the patella show an increased external rotation of the tibia with respect to the femur due to a lack of medial muscular and ligamentary guide.

33 CT-investigations of 23 patients with malignancies of the shoulder girdle were analyzed. In 80% of the surgically treated bone tumours of 17 patients the presence and extend of soft tissue infiltration and of osseous destruction could be delineated. The size of the 6 soft tissue tumours was better demonstrated by CT than by any other radiologic procedure. As CT is the most reliable noninvasive method to visualize the extend of a tumour of the shoulder girdle it is a valuable aid in treatment planning. Diagnostic pitfalls and results of follow-up studies are discussed.

The infection rate of open lower leg fracture is extremely high. Surgical treatment guarantees a reduced risk of infection when compared with conservative treatments. But even osteosynthetic methods such as screw or compression plate fixation show unsatisfactory results because of the additional traumatization of the primarily injured soft tissue combined with a reduction of blood circulation. From July 1973 till September 1976 we treated 57 2nd and 3rd degree open shank fractures with osteosynthetic methods: 46 with compression plate osteosynthesis and 11 primarily or secondarily with external fixation. The osteitis-rate was 14%, in addition to that there was a soft-tissue infection rate of 5%. In order to change our therapeutical procedure we stabilized 2nd and 3rd degree open lower limb fractures and lower leg fractures complicated by soft-tissue damages consequently by external fixation. From October 1976 till May 1978 we treated 39 open shank fractures; 24 of them were 2nd and 3rd degree open fractures. They were treated by external fixation. This change in our surgical treatment resulted in an osteitis-rate of 2.6%, there was no case of soft-tissue inflammation. These results underline the superiority of treating 2nd and 3rd degree open lower leg fractures and fractures combined with soft-tissue injuries consequently and chiefly with AO external fixation.
