Zusammenfassung Das Thema Wasser gewinnt angesichts des fortschreitenden Klimawandels an Bedeutung. Für die menschliche Gesundheit ist der Schutz wichtiger Trinkwasserressourcen von hoher Relevanz, aber auch andere Funktionen der Gewässer, wie die als Erholungsräume. Die Gewässerbewirtschaftung muss sich an die veränderten Bedingungen anpassen und gleichzeitig die europäischen Vorgaben zum Gewässerschutz erfüllen. Der Gesundheitsschutz sollte eine zentrale Triebfeder in diesem Prozess sein.

Background: The SARS-CoV-2 pandemic and climate change are two simultaneously occurring large scale environmental health crises. This provides an opportunity to compare the risk perception of both crises in the population. In particular, whether experiencing the acute pandemic sensitizes people to the risks of ongoing climate change. Methods: Panel participants answered a web-based questionnaire. The risk perception of SARS-CoV-2 and influencing factors were assessed. Differences of risk perception dimensions regarding SARS-CoV-2 and climate change were analyzed as well as associations between dimensions. Results: The results show that an economic impact by the pandemic is associated with more dimensions of SARS-CoV-2 risk perception than an experienced health impact. Moreover, dimensions of risk perception of the pandemic and climate change are perceived differently. Furthermore, the affective dimension of pandemic risk perception is significantly associated with all dimensions of climate change risk perception. Conclusions: Emotional-based coping with the risks of SARS-CoV-2 is associated with risk perception of climate change as well as various factors that shape the individuals’ risk perception. It is currently necessary and will be increasingly necessary in the future to solve coexisting crises, not selectively, but in a common context within the framework of a social-ecological and economic transformation.

Urban residency is associated with exposure to environmental factors, which can influence health in many ways. Neuroscientific research, as well as Public Health research, aim towards broadening evidence in the field of Urban Health. However, it is unclear whether the association between urban living and mental illnesses is causal rather than explainable by other selective effects. This review seeks to gather information on the current evidence regarding urban living and neurological outcomes to demonstrate how Public Health and Neuroscience could complement each other in the field of Urban Health. A scoping review was conducted in four electronic databases according to the PRISMA-statement guidelines. 25 empirical studies were included. Outcomes such as schizophrenia and psychotic disorders, social and cognitive functioning were scrutinised. Evidence was found for alteration of brain functioning and brain structure. Most studies researching cognitive functioning or cognitive decline displayed possible protective effects of urban living compared to rural living. The different study designs in Public Health and Neuroscience could profit from each other. Although the comparability of studies is limited by the inconsistent assessments of urbanity. Synergies and potentials to combine aspects of Public Health and Neuroscience in the field of Urban Health to improve population health became apparent.

The specialist field of "pneumology" is still underrepresented in university clinics in Germany, but this is not the case at the newly founded medical faculty Ostwestfalen-Lippe (OWL) in Bielefeld. This is linked to representing pneumology and internal intensive care medicine in patient care, teaching and research across the board and the opportunity to actively help shape the development of the human medicine faculty in an exciting environment.The early anchoring of the subject "Pneumology" in the model degree program of medical school in OWL (begin winter semester 2021/22) contributes to further visibility and a university medical orientation. In this overview various issues of Pneumology in the Model Degree Program are explored by basic scientists, clinical teachers, members of the medical faculty and a student.In today's Evangelisches Klinikum Bethel (EvKB), pulmonary medicine has a long tradition. The hospital's first lung and infection center was opened in 1927. The EvKB's department for internal medicine, pneumology and intensive care medicine, which has been independent since 2009, is becoming a university clinic for pneumology within the medical faculty OWL. Relevant translational and interdisciplinary research can be intensified.There are 30 "Pneumology" teaching units in the model degree program, which are divided into two study sections using different formats, such as lectures, seminars, hands on courses and skills lab. It is represented in particular in the module complex "Circulation and Respiration". The content of the first phase of teaching was carried out by a module commission, with members representing the subjects involved in the module.Knowledge of the basics from, for example, physiology, pathophysiology, anatomy and pathology are taught to the students in the run-up to the pneumology course. Using the example of physiology, the presentation of the learning content of a basic subject is elaborated in this article.Half of all teaching units on pneumology of the entire course took place in the 2nd semester (in March and April 2022), so that students experienced the clinical relevance of the content at an early stage. There was a particular focus on obstructive airway and restrictive lung diseases. After imparting the basic knowledge of the physical examination of the lungs in the Skills Lab, the most important pathological findings in the above-mentioned diseases on inspection, palpation, auscultation and percussion are demonstrated and practised in patients as part of bedside teaching under supervision.Communication training is also longitudinally integrated into the modular teaching, with a total of more than 200 teaching hours and is performed interdisciplinary. In the "Circulation and Breathing" module eight hours are devoted to this with simulated patients, the anamnesis and therapy advice on classic cardiopulmonary diseases. Damit verbunden ist die Aufgabe, die Pneumologie und internistische Intensivmedizin in Lehre, Krankenversorgung und Forschung in ganzer Breite zu vertreten und die Möglichkeit, in einem spannenden Umfeld den Aufbau der humanmedizinischen Fakultät aktiv mitzugestalten.In diesem Artikel werden verschiedene relevante Facetten von jeweils unterschiedlichen Autor*innen aus medizin-theoretischer und klinischer Perspektive übersichtsartig dargestellt.Im heutigen Evangelischen Klinikum Bethel (EvKB) hat die Lungenheilkunde eine lange Tradition. Bereits im Jahr 1927 wurde das erste Lungen- und Infektionshaus des Klinikums eröffnet. Die seit 2009 eigenständige „Klinik für Innere Medizin, Pneumologie und Intensivmedizin“ des EvKB wird die universitäre Klinik für Pneumologie innerhalb des Universitätsklinikum OWL.Die frühzeitige Verankerung des Faches „Pneumologie“ im Modellstudiengang Medizin (Start Wintersemester 2021/22) trägt zur weiteren Sichtbarkeit und einer universitätsmedizinischen Ausrichtung bei. Das Fach „Pneumologie“ verfügt im Modellstudiengang im Verlauf des Studiums über 30 Unterrichtseinheiten, die sich auf zwei Studienabschnitte und verschiedene Unterrichtsformate verteilen. Die konkrete Festlegung der Lehrinhalte des 1. Studienabschnitts erfolgte in einer Modulkommission, die mit Vertreter*innen der beteiligen Fächer besetzt war.Wissen zu den benötigten Grundlagen aus bspw. der Physiologie, Pathophysiologie, Anatomie und Pathologie wird den Studierenden im zeitlichen Vorfeld der Lehrveranstaltungen der Pneumologie vermittelt. Am Beispiel der Physiologie wird in diesem Artikel die Präsentation von Lerninhalten eines Grundlagenfaches erläutert. Die Hälfte aller Unterrichtseinheiten zur Pneumologie des gesamten Studiums findet bereits im 2. Semester (erstmals im März und April 2022) statt, damit die Studierenden frühzeitig den klinischen Bezug der Lerninhalte erfahren. Besondere Schwerpunkte sind obstruktive Atemwegs- und Lungenerkrankungen sowie restriktive Lungenerkrankungen. Nach Vermittlung der Grundkenntnisse zur körperlichen Untersuchung der Lunge im „Skills Lab“ werden im Rahmen von Unterricht am Krankenbett unter kompetenter Supervision die wichtigsten pathologischen Befunde der Inspektion, Palpation, Auskultation und Perkussion bei Patienten mit den o. g. Erkrankungen erhoben.Auch die Kommunikationsausbildung ist in die modulare Lehre des Studiengangs mit insgesamt mehr als 200 Unterrichtsstunden longitudinal integriert. Sie greift im Themenkomplex Kreislauf und Atmung in insgesamt acht Unterrichtsstunden fächerübergreifend und z. T. mit Simulationspersonen die Anamneseerhebung und Therapieberatung zu klassischen kardiopulmonalen Erkrankungen auf. Aus der Perspektive der Studierenden stellt im Modellstudiengang die Vermittlung des kompakten fachübergreifenden und ineinandergreifenden Wissens zwischen den Grundlagenfächern und klinischen Fächern zum jeweiligen Organ eine Herausforderung dar, wobei die Vorteile aus heutiger Sicht überwiegen.

Zusammenfassung Ziel der Studie Ziel der Studie war die Bereitstellung von Ergebnissen zur Nutzung von Teletherapie während der Corona-Pandemie durch Vertreter*innen von drei Gesundheitsfachberufen. Methoden Im Rahmen einer fragebogengestützten Online-Erhebung wurden 282 Beschäftigte der Ergotherapie, Physiotherapie und Sprachtherapie/Logopädie zu Nutzungsverhalten, Herausforderungen, Potentialen und Rahmenbedingungen von Teletherapie befragt. Ergebnisse Insbesondere Ergotherapeut*innen und Logopäd*innen nutzen die Therapieform. Teletherapie bietet aus Sicht der Befragten auch unabhängig von der Corona-Pandemie Potenzial als alternative Therapieform. Allerdings ist der Bedarf an Unterstützungsangeboten und Schulungen im Bereich der Teletherapie groß. Schlussfolgerung Für eine langfristige Etablierung der Therapieform sind neben technischen Anforderungen und Schulungsangeboten insbesondere Klärungen zu zukünftigen Abrechnungsformalitäten nötig.

Zusammenfassung Ziel der Studie Zusätzlich zur individuellen Gesundheitskompetenz der NutzerInnen spielt die organisationale Gesundheitskompetenz (OGK) von Gesundheitsorganisationen eine große Rolle. In Deutschland sind die Gesundheitsämter (GA) für die Gesundheit auf Bevölkerungsebene zuständig. Sie sollten ihren NutzerInnen helfen, auf Informationen und Dienste zuzugreifen, diese zu verstehen, zu navigieren und für gesunde Entscheidungen zu nutzen. Diese Studie beschäftigt sich erstmals mit der OGK deutscher GA. Welches konzeptionelle Modell beschreibt diese am besten? Methodik Für diese explorative Studie wurden elf leitfadengestützte ExpertInneninterviews mit MitarbeiterInnen von GA in drei deutschen Bundesländern durchgeführt und die Transkripte einer qualitativen Inhaltsanalyse unterzogen. Ergebnisse Für die OGK der GA in Deutschland wurde ein neues konzeptionelles Modell mit zwölf Attributen erstellt. Diese lassen sich den Aspekten „Führung und Organisationskultur“, „MitarbeiterInnen“ sowie „Erstellung und Implementierung von Informationen und Angeboten“ zuordnen. Schlussfolgerungen Das vorliegende Modell für OGK-GA legt den Grundstein für zukünftige Forschung in diesem Bereich. Es ermöglicht die Entwicklung eines Selbstbewertungswerkzeuges und damit perspektivisch die Messung des Kompetenzlevels sowie die Identifikation von Barrieren. Deren Überwindung und damit die Erhöhung der OGK deutscher GA kann erheblich zur Verbesserung der öffentlichen Gesundheit beitragen.

Refugees and asylum seekers face a variety of legal, structural, administrative, cultural and linguistic barriers in accessing healthcare. However, there is currently a lack of available data on the health needs of refugees and asylum seekers. In particular, their subjective experience in accessing healthcare has not been considered sufficiently.This paper explores the subjective experiences of refugees and asylum seekers in accessing and using healthcare. Strategies for dealing with potential challenges are presented.The recruitment of interview partners was based on a quantitative cross-sectional study collected as part of a study on the health of refugees (FlüGe health study). Persons who agreed to be contacted again were contacted by telephone. A heterogeneous sub-sample (n = 18) with regard to age, gender, nationality, health status and utilisation behaviour was interviewed using interpreter-supported problem-centred interviews. Data were analysed using a deductive-inductive approach.The migration-insensitive healthcare for refugees and asylum seekers is characterised by language barriers, disorientation, experiences of rejection, mistreatment and structural barriers. Experiences of foreignness in the healthcare system can lead to delayed care seeking or underusage of healthcare systems. At the same time, individual strategies by refugees, asylum seekers and healthcare providers are used in order to meet these challenges.HINTERGRUND: Menschen mit Fluchtgeschichte sind mit einer Vielzahl von rechtlichen, strukturellen, administrativen, kulturellen und sprachlichen Barrieren im Zugang zur Gesundheitsversorgung konfrontiert. The SARS-CoV-2 pandemic has changed lives around the world. In particular, healthcare workers faced significant challenges as a result of the pandemic. This study investigates the seroprevalence of SARS-CoV-2 in March-April 2020 in Germany among healthcare workers and relates it to questionnaire data. In June 2020, all employees of the reporting hospital were offered a free SARS-CoV-2 antibody test. The first 2,550 test results were sent along with study documents. The response rate was 15.1%. The COVID-19 PCR test prevalence amongst health care workers in this study was 1.04% (95% CI 0.41-2.65%), higher by a factor of 5 than in the general population (p=0.01). The ratio of seroprevalence to PCR prevalence was 1.5. COVID-19-associated symptoms were also prevalent in the non-COVID-19-positive population. Only two symptoms showed statistically significant odds ratios, loss of smell and loss of taste. Health care workers largely supported non-pharmaceutical interventions during the initial lockdown (93%). Individual behavior correlated significantly with attitudes toward policy interventions and perceived individual risk factors. Our data suggest that healthcare workers may be at higher risk of infection. Therefore, a discussion about prioritizing vaccination makes sense. They also support offering increased SARS-CoV-2 testing to hospital workers. It is concluded that easier access to SARS-CoV-2 testing reduces the number of unreported cases. Furthermore, individual attitudes toward rules and regulations on COVID-19 critically influence compliance. Thus, one goal of public policy should be to maintain high levels of support for non-pharmaceutical interventions to keep actual compliance high.

Diese Synergieeffekte müssen genutzt werden, um eine faire und effiziente Allokation von sauberem Wasser zu ermöglichen.

Half of all teaching units on pneumology of the entire course took place in the 2nd semester (in March and April 2022), so that students experienced the clinical relevance of the content at an early stage. There was a particular focus on obstructive airway and restrictive lung diseases. After imparting the basic knowledge of the physical examination of the lungs in the Skills Lab, the most important pathological findings in the above-mentioned diseases on inspection, palpation, auscultation and percussion are demonstrated and practised in patients as part of bedside teaching under supervision. Communication training is also longitudinally integrated into the modular teaching, with a total of more than 200 teaching hours and is performed interdisciplinary. In the “Circulation and Breathing” module eight hours are devoted to this with simulated patients, the anamnesis and therapy advice on classic cardiopulmonary diseases. For the students, integrating the teaching of basic theory and its clinical application for each organ systems represents a challenge in the model degree program, the advantages outweigh from todayʼs perspective.

Background: Strengthening individual health literacy and knowledge about health challenges are important preventive approaches in public health to improve health equality. Health care organizations have come into focus in this regard. They need to raise their organizational health literacy (OHL) to help users to access and navigate information and services. Objective: In Germany, public health departments (PHDs) are responsible for public and environmental health at the population level. This study breaks new ground as it is the first to investigate the OHL of these health care organizations. The study attempts to answer what barriers keep them from raising their OHL and how can these be overcome? Methods: In this explorative study, 10 guideline-oriented interviews with experts from PHDs were conducted in two states in Germany. Qualitative content analysis was used to extract the results from the experts´ statements. Key Results: Eleven barriers to raising OHL were identified. Obstacles were found in leadership and organizational culture, design and implementation of information, and in human resources. False expectations due to negative preconceptions about public service were identified as a new barrier that had not been elucidated in prior literature. The same applies to lack of cooperation on cross-sectoral topics as well as lack of accessibility. Clear communication of tasks and clear jurisdictional authority are some of the recommendations for lowering these barriers. Other examples include identifying and using synergies and involvement of target groups. Conclusions: Overcoming these obstacles could improve the OHL of German PHDs. This, in turn, could improve the health of the general population and thus contribute significantly to overall public health. Extrapolating to the whole of Germany and other countries could further strengthen research on OHL. This competence is called organizational health literacy (OHL). Ten experts from German PHDs were interviewed about barriers to raising OHL. This article reports the barriers identified and recommendations for lowering them, as well as three previously unknown obstacles.

Background: Refugees and asylum seekers have a high prevalence of psychiatric disorders such as posttraumatic stress disorder (PTSD), anxiety and depression. Associations between the number of traumatic events and psychiatric disorders have been reported in literature, but knowledge on the influence of post-migration stressors on mental health outcomes in host countries remains scarce. Method: Four databases in the field of public and mental health were systematically searched. From a total of 2,456 studies found, 1,777 were included for title and abstract screening and 61 full texts were separately screened for eligibility by 2 authors. Out of these studies, 23 studies were included in this systematic review. A narrative synthesis was undertaken.Results: The overall findings showed that post-migration stressors have a significant impact on mental health outcomes. Especially uncertain legal status, missing social support and unemployment were shown to negatively influence mental health, but results vary among the different psychiatric disorders, i.e. post-traumatic stress disorder, depression and anxiety. Discussion: Post-migration stressors can contribute to the high prevalence rates of psychiatric disorders in refugees in European high-income countries. However, results must be interpreted with caution because of the heterogeneous study populations included and the variety of post-migration stressors. None of the studies considered the effect of mental disorders on the perceived severity of post-migration stressors.

The aim of the study was to investigate the use of teletherapy during the corona pandemic by three non-medical therapy professionals in the health sector.As part of a questionnaire-based online survey, 282 participants from the field of ergotherapy, physiotherapy and speech therapy were asked about usage behavior, challenges, potentials, and general conditions of teletherapy.Especially ergo and speech therapists used teletherapy during the corona pandemic. From their point of view, teletherapy also had a potential to be used as an alternative form of therapy, regardless of the coronavirus pandemic, adding that there was a great need for further assistance and training in the field of teletherapy.To implement this form of therapy on a long-term basis, in addition to technical requirements and training opportunities, accounting formalities need to be clarified.Ziel der Studie war die Bereitstellung von Ergebnissen zur Nutzung von Teletherapie während der Corona-Pandemie durch Vertreter*innen von drei Gesundheitsfachberufen.Im Rahmen einer fragebogengestützten Online-Erhebung wurden 282 Beschäftigte der Ergotherapie, Physiotherapie und Sprachtherapie/Logopädie zu Nutzungsverhalten, Herausforderungen, Potentialen und Rahmenbedingungen von Teletherapie befragt.Insbesondere Ergotherapeut*innen und Logopäd*innen nutzen die Therapieform. Teletherapie bietet aus Sicht der Befragten auch unabhängig von der Corona-Pandemie Potenzial als alternative Therapieform. Allerdings ist der Bedarf an Unterstützungsangeboten und Schulungen im Bereich der Teletherapie groß.Für eine langfristige Etablierung der Therapieform sind neben technischen Anforderungen und Schulungsangeboten insbesondere Klärungen zu zukünftigen Abrechnungsformalitäten nötig.

In addition to the users' individual health literacy (HL), the organizational health literacy (OHL) of health care organizations plays an important role as well in assuring the health of the poipulation. In Germany, public health departments are responsible for public health at population level. They should help their users to access, understand and navigate and use information and services in their health-related decisions. This study addresses the question of the best conceptual model of OHL of the German public health departments for the first time.In this explorative study, eleven guideline-oriented interviews with experts from public health departments were conducted in three German federal states. Subsequently, the transcripts underwent qualitative content analysis.A new conceptual model consisting of twelve attributes was identified for OHL of public health departments in Germany. These twelve characteristics can be assigned to the aspects "leadership and organizational culture", "employees" and "development and implementation of information and services".This model for the OHL-PH lays the foundation for future research in this area. It enables the development of a self-assessment tool and therefore the measurement of the level of OHL as well as the identification of barriers. Increasing the OHL of German public health departments by overcoming these obstacles can make a significant contribution to improving public health.Zusätzlich zur individuellen Gesundheitskompetenz der NutzerInnen spielt die organisationale Gesundheitskompetenz (OGK) von Gesundheitsorganisationen eine große Rolle. In Deutschland sind die Gesundheitsämter (GA) für die Gesundheit auf Bevölkerungsebene zuständig. Sie sollten ihren NutzerInnen helfen, auf Informationen und Dienste zuzugreifen, diese zu verstehen, zu navigieren und für gesunde Entscheidungen zu nutzen. Diese Studie beschäftigt sich erstmals mit der OGK deutscher GA. Welches konzeptionelle Modell beschreibt diese am besten?Für diese explorative Studie wurden elf leitfadengestützte ExpertInneninterviews mit MitarbeiterInnen von GA in drei deutschen Bundesländern durchgeführt und die Transkripte einer qualitativen Inhaltsanalyse unterzogen.Für die OGK der GA in Deutschland wurde ein neues konzeptionelles Modell mit zwölf Attributen erstellt. Diese lassen sich den Aspekten „Führung und Organisationskultur“, „MitarbeiterInnen“ sowie „Erstellung und Implementierung von Informationen und Angeboten“ zuordnen.Das vorliegende Modell für OGK-GA legt den Grundstein für zukünftige Forschung in diesem Bereich. Es ermöglicht die Entwicklung eines Selbstbewertungswerkzeuges und damit perspektivisch die Messung des Kompetenzlevels sowie die Identifikation von Barrieren. Deren Überwindung und damit die Erhöhung der OGK deutscher GA kann erheblich zur Verbesserung der öffentlichen Gesundheit beitragen.

What shapes farmers perception of agricultural wastewater reuse (WWR) in Palestine under circumstances of water shortage, poor sanitation, and political uncertainty? The current study answers this question by applying the ‘cultural approach’ to situate farmers’ perceptions of agricultural WWR within broader contextual frameworks and offer interpretations beyond individual attitudes provided solely by the psychological and psychometric approaches. It deploys qualitative case study research design and combines six qualitative and quantitative data collections. The study argues that a complex series of interrelated individual and contextual factors shape farmers’ perceptions of agricultural WWR. While farmers’ economic, agricultural, health, environmental, and political concerns related to agricultural WWR significantly influence their perceptions of using RW, farmers’ psychological, social, and religious concerns were not significant. Contrary to what has been perceived by stakeholders in the Palestinian water and wastewater sector, the researchers conclude that the issue of social acceptance of agricultural WWR is not an obstacle to implementing agricultural WWR projects in Palestine. Institutional conservatism, which resists community participation, and weak institutional structure could be what initially contributed to farmers’ opposition to past WWR projects. The researchers contribute both conceptually and methodologically to the existing knowledge. They offer a conceptual model for case study research examining farmers’ perceptions of agricultural WWR and invent the concept of Case-Driven Significant Factor (CDSF) which promotes a new type of individual and contextual factors derived from the particularities of the case itself.

E-Health-Leistungen erstrecken sich vom Bereich der onlinebasierten Gesundheitskommunikation uber die intersektorale und interprofessionelle Vernetzung der Akteure im Gesundheitswesen bis hin zur Anwendung von Informations- und Kommunikationstechnologien in der direkten medizinischen Versorgung von Patientinnen und Patienten. Die Implementation von E-Health-Leistungen ist dabei ein vordergrundiges Ziel gesundheitspolitischer Entwicklungsprozesse mit der Absicht, speziell telemedizinische Leistungen flachendeckend in Europa zur Verfugung zu stellen. Dabei sieht sich die Implementation von E-Health als Teil von Pravention, Gesundheitsforderung und medizinischer sowie pflegerischer Versorgung mit unterschiedlichen Herausforderungen konfrontiert. Neben medizin- und sozial-ethischen Fragestellungen sind dies insbesondere Aspekte der Schaffung rechtssicherer Rahmenbedingungen, die Vergutung und Finanzierung der Technologien, die Gestaltung der Technik mit Blick auf eine nutzerinnen- und nutzerorientierten Usability, die Einbeziehung der Technik in die Versorgungsprozesse entlang der Bedarfe und Bedurfnisse der Nutzerinnen und Nutzer, die Gewahrleistung technologischer Interoperabilitat durch einheitliche Standards oder die Wirksamkeitsanalyse von E-Health, sowohl in Bezug auf gesundheitliche als auch okonomische Endpunkte. Der vorliegende Beitrag soll das breite Spektrum von E-Health-Leistungen im Gesundheitswesen systematisieren, Grundlagen der technologie-gestutzten Kommunikation im Gesundheitswesen vermitteln und dabei ausgewahlte rechtliche Rahmenbedingungen in den Blick nehmen. Daruber hinaus werden die ubergeordneten Potenziale der Nutzung von E-Health-Leistungen herausgearbeitet, praktische und ethische Problembereiche betrachtet und auf dieser Grundlage die Implementationsherausforderungen aus unterschiedlichen Perspektiven diskutiert.

ZusammenfassungPublic Health hat vielfältige Möglichkeiten, gesundheitlichen Folgen des Klimawandels proaktiv zu begegnen. Neben Forschung zu Auswirkungen klimatischer Veränderungen, Nebeneffekten von Klimaschutz- und zur Wirksamkeit von Anpassungsmaßnahmen kommt Public Health-Expert*innen eine bedeutende Rolle in Prävention und Behandlung von klimawandelbedingten Gesundheitsstörungen zu. Erforderliches Wissen gilt es in Public Health- und Medizin-Curricula zu vermitteln. Weitere Aufgaben sind systematische Reduktionen von CO2-Emissionen im Gesundheitssektor und Umsetzung von Klimaanpassungsmaßnahmen.

Durch den Einsatz von Sprachmittler_innen ist die qualitative Forschung mit einer dreifachen Subjektivitat konfrontiert, die methodische Anpassungen bei Datenerhebung und -auswertung notwendig macht. Anhand der (2009) formulierten vier Kriterien „Konzeptionelle Aquivalenz“, „Qualifikation und Referenzen der Ubersetzer_innen“, „Rolle des/der Sprachmittler_in im Ubersetzungsprozess“ und „Anpassung an den Forschungsgegenstand“ wird das Vorgehen in einer qualitativen Interviewstudie mit Gefluchteten zum Erleben von Gesundheit und Krankheit in der fremden Lebenswelt reflektiert. Die Autorinnen arbeiten Beispiele fur gelungene und herausfordernde Dolmetschsituationen heraus und stellen diese anhand einzelner Interviewpassagen dar. Daraus werden abschliesend Handlungsempfehlungen fur den Einsatz von Sprachmittler_innen abgeleitet.

Die umfassende Berucksichtigung von Geschlecht (biologische und soziale Dimensionen) in der Gesundheitsforschung ist essenziell, um Relevanz und Validitat der Forschungsergebnisse zu erhohen. Im Unterschied zu anderen Bereichen der Gesundheitsforschung fehlt bislang eine systematische Aufarbeitung des Forschungsstandes zur Bedeutung von Geschlecht bei umweltbezogener Gesundheit. In dem interdisziplinaren Forschungsnetzwerk Geschlecht – Umwelt – Gesundheit (GeUmGe-NET) wurde in ausgewahlten Themenfeldern der Forschungsbereiche Umwelttoxikologie, Umweltmedizin, Umweltepidemiologie und Public-Health-Forschung zu Umwelt und Gesundheit der aktuelle Stand zur Integration von Geschlechteraspekten bzw. Genderkonzepten systematisch erhoben. In allen Bereichen wurden Wissenslucken und Forschungsbedarfe identifiziert sowie Potenziale fur methodische Weiterentwicklungen durch Nutzung von Genderkonzepten aufgezeigt. Mit dem Forschungsnetzwerk GeUmGe-NET wurde ein Dialog zwischen biomedizinisch bzw. Public-Health-orientierter Gesundheitsforschung und sozialwissenschaftlicher Genderforschung begonnen. Dieser Dialog ist weiterzufuhren, insbesondere hinsichtlich der gemeinsamen Erprobung methodischer Innovationen bei Datenerhebung und Datenanalyse. Erkenntnisse aus dieser interdisziplinaren Forschung sind relevant fur die Praxisfelder umweltbezogener Gesundheitsschutz, Gesundheitsforderung, Umweltgerechtigkeit und umweltbezogene Gesundheitsberichterstattung.

Current international studies show that environment-related diseases disproportionately affect vulnerable people. This is a case of environmental injustice. Environmental justice goes beyond the mere description of environment- and health-related social inequalities by comprising two dimensions of justice as a normative approach: distributional and procedural justice. Attempts to explain the link between social circumstances, the environment and health deal with both the socially unequal distribution of environmental hazards and environmental resources (exposure variation) and social differences in vulnerability to the health effects of environmental exposures (effect modification). Integrated monitoring approaches provide the basis for deriving interventions under various aspects of environmental justice. Parting from public health research and embedded in the Health in All Policies (HiAP) concept, environmental justice has now been taken up in a number of fields, including politics, administration and practice. There are strategic considerations and attempts to anchor it in politics at the federal, state and the communal level, both by government and non-government groups. Health-promoting urban development is a core field for intervention. The Soziale Stadt (Social City) programme for promoting urban planning and construction as well as place oriented sectoral planning make an important contribution by helping to focus on urban spaces with multiple health hazards and to implement target group-oriented participation processes. There continues to be a need to develop methods and systematically implemented evaluations of political strategies and corresponding interventions regarding their effects on inequalities in health and environmental justice.

A cross-sectional analysis among 1041 urban residents was done to examine associations between self-reported walking distance to and use of freshwater urban blue space and health-related quality of life in two German cities: a city with “poor” urban blue space supply (Bielefeld; 0.8% blue space) and one with “better” urban blue space supply (Gelsenkirchen; 3.0% blue space). Health-related quality of life was assessed with the SF-12v2, which measures residents’ self-reported mental and physical health. Results showed a significant association between use frequency and perceived walking distance to blue space. Blue space use was a better indicator of health outcomes than perceived walking distance. After controlling for green space, socio-economic and demographic variables, use frequency was associated with higher mental health in Gelsenkirchen. These cross-sectional findings suggest that blue space use increases the probability of being healthier in highly urbanised areas in cities with an appropriate amount of blue space.

ZIEL DER STUDIE: Umweltrisikofaktoren haben häufig eine hohe gesundheitliche Relevanz mit einem großen und z. T. nicht ausreichend genutzten Präventionspotenzial. Das Wissen über die komplexen Wirkungszusammenhänge zwischen Umwelt und Gesundheit ist für einige Umweltrisikofaktoren begrenzt. Ein Instrument, das bestehende Evidenz nutzt, um die gesundheitlichen Auswirkungen für eine Bevölkerung oder Bevölkerungsgruppe zu quantifizieren, ist das Konzept der umweltbedingten Krankheitslast (Environmental Burden of Disease, EBD). Ziel des Beitrages ist es, das Konzept der umweltbedingten Krankheitslast vorzustellen sowie den Nutzen aber auch die Kritikpunkte zu benennen. METHODIK: EBD fasst Morbiditäts- und Mortalitätsdaten in einem Summenmaß, dem Disability-Adjusted Life Year (DALY), zusammen und ermöglicht eine vergleichende Darstellung von Krankheitslasten. Hierfür werden Lebensjahre als Einheit genutzt. Mithilfe des attributablen Anteils kann ermittelt werden, welcher Anteil der Gesamtkrankheitslast auf umweltbedingte Risikofaktoren zurückzuführen ist. ERGEBNISSE: Das EBD-Konzept wird, trotz seiner zunehmenden Anwendung im internationalen Raum, häufig kritisch diskutiert, da die Einschränkung des mehrdimensionalen Konstrukts Gesundheit in einer Maßzahl zusammengefasst wird. Weitere Kritik bezieht sich auf die sozialen und normativen Konventionen, die das Konzept beinhaltet. Fehlende oder unzureichende Eingangsdaten können zudem die Qualität und Aussagekraft von EBD-Schätzungen limitieren. SCHLUSSFOLGERUNG: Es bedarf eines wissenschaftlichen Diskurses, in welchem Rahmen das Konzept in Deutschland genutzt werden kann und sollte. GOAL OF THE STUDY: Environmental risk factors are of great importance for public health with a considerable but often unused potential for prevention. However, knowledge about the complex associations between the environment and health effects is limited for some risk factors. A concept, which is using the existing evidence on associations between the impact of environmental factors and health effects, is the environmental burden of disease (EBD) concept. The aim of this article is to present the quantification method of the EBD concept and to discuss the advantages and its points of criticism. METHODS: The EBD concept combines morbidity and mortality data in a single measure (Disability-Adjusted Life Year, DALY) to enable a comparative description of the burden of disease. Life years are used as measurement unit. The environmental share of the total DALYs is quantified by using the attributable fraction. RESULTS: Despite its increasing application especially in the international context, the method is still criticized, because by summarizing the complex construct of health in one single measurement unit, much important information about quality of life is lost. A further criticism refers to partly arbitrarily set social value choices. Additionally, missing or insufficient data can limit the quality and validity of EBD estimations. CONCLUSION: A scientific discourse is needed to decide to what extent the EBD approach can and should be used in Germany.

In addition to lowering the quality of life of those affected, long periods of treatment and high recurrence rates of chronic wounds cause major economic costs for health care systems. Furthermore, inadequate health care and undersupply of care can be observed in Germany. Thus, new forms of care such as teleconsultation are being discussed increasingly. Recent changes in the remuneration system and macerations in the ban of remote treatment support those trends.A systematic review was conducted in July 2018 using PubMed and CENTRAL databases for randomized controlled trials between 2008 and 2018. Only randomized clinical trials in which patients with chronic wound who received treatment and follow-up by teleconsultation using information and communication technology to share data were included. In total, 6 international clinical trials were identified.Regarding the current state of research, there is no definite evidence that teleconsultation can improve the care of chronic wound patients. Most likely, wound healing time was positively influenced. No correlation was found between mortality and hospitalizations when teleconsultation was used.On the basis of the included studies, the evidence for the care of chronic wounds by teleconsultation is unclear. Further research is needed to examine the health-related and economic benefits of teleconsultation to support integration into health care systems.

The supply of online therapy options for mild to moderate depressive disorders has substantially risen both nationally and internationally in the past years. While the efficacy and efficiency of specific online-based therapeutic services were indicated within the framework of scientific evaluations, little is known about the acceptance of these new therapy options.In a nationwide online survey of online-based therapy options for mild to moderate depressive disorders, the attitude and stance as well as the use intention and the subjectively perceived knowledge of 657 medical practitioners and therapists, active in psychotherapeutic and psychiatric treatment were collated. The impact of the potential predictors on the use intention of online-based therapies, based on the unified theory of acceptance and use of technology, was analyzed by means of a binary logistic regression model.Besides the perceived performance and expense expectations, the supportive framework conditions, the influence of the cooperative environment, the individual technical affinity as well as the evaluation of the prospective significance of online-based therapy proved to be significantly influential predictors on the user intention. Special predictability for a positive user intention was shown related to the perceived potential of online therapy.The results underline the relevance of further evidence-basing of online therapy for mild to moderate depressive disorders. A user-oriented, participatory technology development proves to be just as relevant as a comprehensive sensitization, elucidation and education of potential users for a successful implementation.

Background To master challenges to their environmental health, people have to have health literacy (HL). At a national and communal level, focus is shifting from individual HL to health care organizations. They have to raise their organizational health literacy (OHL) to help their users to access information and services. In Germany, public health departments are responsible for environmental health at a population level while the environmental administration is responsible for health-related environmental protection. To date, there is no research on the OHL of public health departments in Germany. Which are the barriers that keep public health organizations from raising their OHL as well as from making better use of possible synergies with the environmental administration? Methods The study consisted of interviews with experts from public health (n = 7) and environmental departments (n = 5) at communal (n = 8) up to federal state level (n = 4). Experts were asked for their insight on barriers for OHL as well as for cross-sector collaboration. They were selected for a wide range in socioeconomic factors and administrative levels. The interviews were recorded between 10/2018-04/2019. After transcription the anonymized interviews underwent qualitative content analysis. Results In twelve interviews 10.4 h of audio were recorded. Content analysis revealed 13 barriers for OHL. Several of those had been described for hospitals before while others, like low responsiveness to the needs of the public, were unknown so far. Preliminary results also indicated several barriers for cross-sectoral cooperation as well as recommendations to overcome said barriers. Conclusions Interviews with public health and environmental administrative organizations turned up 13 barriers that keep them from raising their OHL. Implementing preliminary recommendations could result in better cooperation, in higher OHL, higher environmental health and therefore contribute to public health in the general population. Zusammenfassung Der dritte Teil des DGP-Positionspapiers stellt den aktuellen Wissensstand zu bislang weniger untersuchten Gesundheitsgefährdungen durch Luftschadstoffe vor: gestörte Glukosetoleranz und Diabetes sowie neurodegenerative Erkrankungen und neurokognitive Entwicklung bei Kindern. Weiterhin wird die Bedeutung einer Exposition während der Schwangerschaft für Mutter und Kind beschrieben und abschließend in die derzeit diskutierten Mechanismen zur Erklärung der unterschiedlichen adversen Effekte von Luftschadstoffen eingeführt. Verschiedene, sich oft ergänzende Pathomechanismen bilden die Grundlage für die unterschiedlichen, durch Luftschadstoffe bedingten Gesundheitseffekte. Oxidativer Stress und eine subklinische entzündliche Reaktion in der Lunge, aber auch auf systemischer Ebene („low-grade systemic inflammation“), stehen dabei im Mittelpunkt. Diese begünstigen sekundäre Veränderungen im Organismus wie vaskuläre oder metabolische Prozesse und können darüber hinaus zu epigenetischen Effekten oder zur „Neuroinflammation“ führen. Die Bedeutung von löslichen, systemisch verfügbaren Partikelbestandteilen aber auch die Translokation von ultrafeinen Partikeln aus der Lunge über die Blutbahn in sekundäre Zielorgane wie Leber, Gehirn oder den Fötus wird dabei intensiv diskutiert. Diabetes mellitus ist eine der häufigsten chronischen Erkrankungen weltweit, mit einer Prävalenz von knapp 14 % in Deutschland. Bei dem mit großem Abstand häufigeren Typ 2-Diabetes mellitus spielen Lebensstilfaktoren bei der Genese eine wesentliche Rolle. Toxikologische und epidemiologische Studien legen darüber hinaus nahe, dass auch eine langfristige Luftschadstoffbelastung zu einem erhöhten Risiko v. a. für den Typ 2-Diabetes beitragen kann. Zusätzliche Hinweise für eine ursächliche Rolle liefern Studien zur Glukoseregulation, der Insulinsensitivität und dem Schwangerschaftsdiabetes. Ergebnisse experimenteller Studien unterstützen diese Zusammenhänge und zeigen plausible biologische Mechanismen auf. Jedoch sind zur Stärkung der gegenwärtigen Evidenz prospektive Studien mit Berücksichtigung multipler Lebensstil- und Umweltfaktoren wie Grünflächen oder Lärm und einer präziseren individuellen Abschätzung der Schadstoffbelastung notwendig. Mit der Altersentwicklung in der Bevölkerung nimmt die Krankheitslast durch neurodegenerative Erkrankungen zu. Erste Studien weisen auf einen möglichen Beitrag durch Luftschadstoffe, v. a. durch Feinstaub, hin. So wird in einigen Studien bei einer erhöhten Schadstoffbelastung eine Abnahme der neurokognitiven Leistungsfähigkeit im Erwachsenenalter und ein erhöhtes Risiko für eine Demenz oder eine Alzheimer-Erkrankung beobachtet, jedoch sind die Studien in Bezug auf Design, Expositionsabschätzung und Gesundheitseffekt noch inhomogen und die Studienergebnisse insgesamt gesehen noch inkonsistent. In Bezug auf die neurokognitive Entwicklung im Kindesalter beschreiben erste Studien einen Zusammenhang zwischen dem Grad der Luftverschmutzung, z. B. an der Schule, und einer verzögerten kognitiven Entwicklung. Auch wenn die Evidenz für die verschiedenen biologischen Endpunkte während der Schwangerschaft noch sehr heterogen ist, weisen die Studien insgesamt auf einen negativen Einfluss der Luftschadstoffe auf den mütterlichen und fetalen Organismus hin. Die stärkste Evidenz liegt für ein verringertes Geburtsgewicht im Zusammenhang mit erhöhten Luftschadstoffen vor, allerdings mit relativ niedriger Effektgröße von im Mittel nur wenigen Gramm. Although lifestyle factors are the main causes, current evidence suggests that long-term exposure to air pollution may additionally increase the risk for type 2 diabetes. Supporting evidence for a causal role of air pollution is provided by studies addressing the regulation of the blood glucose levels in metabolically healthy participants, insulin sensitivity, or pregnancy-related diabetes. Experimental studies provide further support for plausible biological mechanisms. However, prospective studies are needed to gain more evidence, taking multiple lifestyle and environmental factors, such as green space and noise, and an improved individual exposure assessment into account.The aging population has an increased risk of neurodegenerative diseases. First studies point towards a contribution of chronic exposure to air pollution, specifically by particulate matter. Several studies report its association with decreased neurocognitive capacity or an increased prevalence of dementia or Alzheimer's disease in adults. However, the studies are inhomogeneous regarding design, exposure and outcome, leading to inconsistent results. With respect to the influence on neurocognitive development of children, first studies suggest an association between the level of air pollution, e. g. at school, and delayed cognitive development.Even though the evidence for the different biological endpoints during pregnancy is still heterogeneous, the studies generally point towards an adverse impact of air pollution on the maternal and fetal organisms. Hintergrund: Der Einsatz von Informations- und Kommunikationstechnologien in der ambulanten Pflege erfolgt vor dem Hintergrund des demografischen Wandels mit dem Ziel, auch in Zukunft die Qualität der pflegerischen Versorgung sicherzustellen. Die Akzeptanz der telematischen Anwendungen ist dabei entscheidend für die erfolgreiche Implementation. Ziel: Das Ziel der Untersuchung war eine Erfassung der subjektiven Haltungen und Einstellungen sowie der Nutzungsintention der Pflegekräfte hinsichtlich telematischer Anwendungen in der ambulanten Pflege. Methoden: Auf der theoretischen Grundlage der „Unified Theory of Acceptance and Use of Technology“ (UTAUT) wurde eine Onlineumfrage erstellt, anhand derer deutschlandweit ambulante Alten- und Krankenpflegekräfte sowie Auszubildende in Pflegeschulen befragt wurden. Insgesamt konnten 371 Befragte in die Analyse eingeschlossen werden. Ergebnisse: Der Informationsstand zur Telematik und die eigenen technischen Kompetenzen werden als gering eingeschätzt. Issue With most humans, globally, now living in cities, “Urban Health” receives increasing attention. For sustainably securing and promoting human health in the urban setting, a multitude of urban systems, fierce dynamics, and systemic interactions need to be considered. The goal is to provide an integrative view of Sustainable Urban Health (SUH), i.e. capturing the “forest” instead of merely the “trees”. How is this possible, and what insights can be derived? A theory-based SUH approach, starting out from the Human ecology perspective, provides a framework which is compatible with WHO’s Healthy Cities approach. Methods Methods used include literature searches and managed cooperation, proceeding from a model exercise (published paper) to a full-scale book publication, thus building bridges across disciplines and societal sectors. Results As for results, there is a comprehensive account of Hamburg SUH, including history, health status, governance, health care, rehabilitation, prevention and health promotion, “health in all policies” (e.g. economics, housing, mobility), and current integrative community projects. 100 authors delivered c.70 contributions. Activities around “health” are shown to constitute a remarkable element of urban culture, with room for improvement, e.g. concerning quality assurance on a systems level; more focus on sustainability (incl. environmental impact and overall future-proofing); and safeguarding relevant sources as a “collective memory”. To take the issue forward in civic society, one venerable Hamburg NGO (founded in 1765) now established a SUH working group. Conclusions The intensity and diversity as well as certain catch-up needs of SUH in a European metropolis are demonstrated. This “no regrets” approach meets with interest also in civic society. Once it is applied in multiple cities, innovative concepts and efforts can be compared easily, and get locally adjusted, for further strengthening SUH. Key messages Sustainable Urban Health (SUH), based on Human ecology, is an integrative “no regrets” approach and can cooperatively be applied anywhere, informing and guiding professionals and civic society. Even where activities around “health” constitute a remarkable element of urban culture, there is room for improvement, e.g. quality assurance on systems level, and more focus on sustainability.

Zusammenfassung Der Beitrag befasst sich mit der Berichterstattung zur Teilhabe von Menschen mit Behinderungen in Deutschland. Hierbei wird auf die vorhandenen Daten zur gesundheitlichen Lage und Versorgung von Menschen mit Behinderungen und Beeinträchtigungen fokussiert. Weitergehend werden Erfordernisse der zukünftigen Berichterstattung aufgezeigt.

Telematics in ambulatory care: Exploring the nurses' perspective Abstract.In the context of demographic change, the use of information and communications technology in home care has the aim of ensuring the quality of nursing care in the future. The acceptance of telematic applications by all users is crucial in this regard.The aim of this study was to assess the subjective attitudes and intentions of nursing staff on telematic applications in home care.Based on the "Unified Theory of Acceptance and Use of Technology" (UTAUT), an online survey was conducted in which nursing staff as well as nursing students throughout Germany were surveyed. 371 questionnaires have been included in the analysis.Nursing students as well as nurses stated inadequate information and insufficient technical competence. Referring to performance expectations, both groups hope for improving care process transparency and communication inside and outside sectors. For effort expectancy, higher costs, training efforts and technology dependency were named.The results show the relevance of the attitudes and demands of users for a successful implementation. In the future, training and further education should increasingly promote the technological competence of nursing staff.Zusammenfassung. Evidence-based political measures need reliable information about the health status of a population and the determinants affecting health. Here, environment and health indicators can provide helpful additional insights.This article provides an overview of existing indicators in the field of environment and health.There are single indicators and indicator sets describing solely the environment or health as well as some indicators integrating both aspects.The indicator sets cover classical epidemiological indicators but also summary measures of population health, which combine mortality and morbidity as well as simple descriptions of the exposure towards environmental risks. The indicator sets mostly cover water and air quality related aspects. For some of the indicators their influence on health is also presented. Furthermore, environment related health indicators are part of sustainability indicator sets. There are indicators on the international, European, national, and municipal level.All indicator sets aim to support policy-making by advising on measures and setting priorities in the area of environment and health protection. However not all indicators reflect the effect of the environment on health adequately. Therefore, further development of the existing indicators is necessary to reflect current progress (e. g. political needs) and to include new scientific evidence in the field of environment and health. A continuous provision, review, and interpretation of meaningful indicators is required to identify trends and to react to these in order to protect the environment and health. This is necessary to adequately pursue the precautionary principle.

Current international studies show that environment-related diseases disproportionately affect vulnerable people. This is a case of environmental injustice. Environmental justice goes beyond the mere description of environment- and health-related social inequalities by comprising two dimensions of justice as a normative approach: distributional and procedural justice. Attempts to explain the link between social circumstances, the environment and health deal with both the socially unequal distribution of environmental hazards and environmental resources (exposure variation) and social differences in vulnerability to the health effects of environmental exposures (effect modification). Integrated monitoring approaches provide the basis for deriving interventions under various aspects of environmental justice. Parting from public health research and embedded in the Health in All Policies (HiAP) concept, environmental justice has now been taken up in a number of fields, including politics, administration and practice. There are strategic considerations and attempts to anchor it in politics at the federal, state and the communal level, both by government and non-government groups. Health-promoting urban development is a core field for intervention. The Soziale Stadt (Social City) programme for promoting urban planning and construction as well as place oriented sectoral planning make an important contribution by helping to focus on urban spaces with multiple health hazards and to implement target group-oriented participation processes. There continues to be a need to develop methods and systematically implemented evaluations of political strategies and corresponding interventions regarding their effects on inequalities in health and environmental justice.
