La crise sanitaire liée à la pandémie de COVID-19 a conduit la collectivité à s’interroger sur la valeur accordée à la santé, par rapport aux autres dimensions de la vie. Mais dans quelle mesure les économistes disposaient-ils d’outils pour guider les décisions publiques dans ce contexte épidémique ? Certains arbitrages préexistaient à la crise : entre la protection de la santé de la population à court, ou moyen ou long terme, entre la protection de la santé de certaines populations au détriment d’autres, entre la protection de la santé de la population et la satisfaction de besoins extra-sanitaires (loisirs, éducation, etc.). Les outils que les économistes mobilisent pour évaluer les interventions innovantes et coûteuses (médicaments, programmes de dépistage, etc.) peuvent être utilisés pour éclairer le débat démocratique. La crise a toutefois soulevé des arbitrages plus inédits lorsqu’il s’est agi de choisir entre, d’une part, la protection de l’état de santé de la population, et, de l’autre, la protection des libertés individuelles. Est-il possible et légitime de mobiliser les méthodes issues de l’économie du bien-être pour trancher ce dilemme ?

L’arrivée récente de thérapies innovantes et coûteuses dans divers champs thérapeutiques suscite des espoirs importants du point de vue de la santé publique. Offrir un accès équitable à ces innovations entre toutefois en concurrence avec d’autres investissements publics qui font également l’objet d’attentes sociales fortes. Elle contraint donc les acteurs à définir le montant maximum que la collectivité est prête à dépenser pour des gains de santé donnés. Survient alors la question de savoir si ce montant maximum varie en fonction des circonstances qui entourent les individus, notamment la rareté de la maladie, leurs modes de vie, les inégalités sociales qu’ils ont subies tout au long de leur vie... Nous nous intéressons dans le cadre de cet article à une priorité particulière, celle accordée aux populations les plus jeunes et qui est le plus souvent désignée, à la suite des travaux de Harris, sous le terme fair innings. Nous nous demandons ce qui pourrait justifier une telle priorisation. Dans le cadre de cet article nous examinons trois arguments. Dans un premier temps nous proposons de justifier le critère de fair innings au nom d’un objectif d’égalisation des possibilités de bien-être dans la population. Dans un deuxième temps, nous proposons de le justifier au nom d’un objectif d’égalisation du temps offert aux individus pour réaliser leurs plans de vie, conformément à la théorie de la justice comme équité de Rawls. Enfin, nous proposons de le justifier au nom d’un objectif d’égalisation du temps offert aux individus pour accepter la mort. Ces trois arguments présentent bien sûr de nombreuses limites et nous soulignons certaines d’entre elles. L’objectif que nous poursuivons dans cet article n’est pas de convaincre de la supériorité d’un argument par rapport aux autres. Nous cherchons plutôt à contribuer aux discussions sur les critères de priorisation en santé, en explicitant ce qui pourrait justifier un des critères particuliers : celui de l’âge., Financing innovative and costly treatments in various therapeutic fields entails a number of problems in countries where costs are covered by public services. Providing these drugs is forcing actors to define the maximum sums of money society is willing to spend for given health improvements. This raises the question of whether maximum financing should vary according individuals’ circumstances, such as the rareness of a disease, lifestyles, social inequalities experienced over a life time, etc. This article examines a particular priority, namely that given to the youngest patients, such prioritising usually refers to the “fair innings argument” (FIA). We question what could justify such a prioritization criterion. Three arguments are considered. At first we consider that FIA could be justified by the objective of equalizing the opportunities of well-being. Next, we proposed justifying the fair innings argument with the aim of equalizing the time provided to individuals to achieve their plan of life, in accordance with Rawls’s theory of justice as fairness. Finally, we proposed considering the FIA as being justified because of the goal of equalizing the time provided to individuals to accept death. These three arguments, of course, have many limitations, some of which we have highlighted. This article doesn’t aim to convince about the superiority of one argument over the other ones. We seek to contribute to the discussions which could happen on prioritization criteria regarding health care, by making explicit what could justify one specific criterion: the age criterion.

Systematic reviews were conducted on the existence of screening tools for epilepsy, quality of life or comorbidities tools, but not specifically in low- and middle-income countries. This study aimed to identify the different tools developed and validated in low- and middle-income countries for the investigation of epilepsy. This to facilitate research in these regions and to identify needs in areas where few instruments are available.This review was conducted according to the PRISMA (Preferred Reporting Items for Systematic Reviews and Meta-Analyses) guidelines. PubMed, Embase, MedLine, Google Scholar, Web of Science, SciELO, Neurology Asia, African Journal of Neurological Sciences and Institute of Epidemiology and Tropical Neurology bibliographic databases were investigated. Articles were included according to the following criteria: (1) to be validation studies on tool for investigating epilepsy, (2) to be conducted in a low- or middle-income country, (3) to be published between 1980 and 2021, and (4) to be written in English, French, Spanish or Portuguese. The characteristics of the tools and validation methods were collected. The frequency of use of the tools was estimated.Ninety-three articles were retained, corresponding to 91 tools from 44 countries. The main tools targeted quality of life (n = 30), comorbidities (n = 19) and screening (n = 13). Instruments were mainly developed and validated in Asia (n = 43), then in Central and South America (n = 24). The IENT (Institute of Epidemiology and Tropical Neurology) questionnaire was developed in several tropical countries (Africa, South East Asia and Latin America). The development and validation methods were heterogeneous from one tool to another and some tools (e.g., QOLIE-31, NDDI-E, PATE, etc.) were culturally adapted.This review identifies geographic specificity regarding the creation, validation and use of tools for investigating epilepsy available in low- and middle-income countries. It will help investigators in the choice of tools in their future epidemiological studies on epilepsy in this context.

We evaluate the introduction of various forms of antihypertensive treatments in France with a distribution-sensitive cost-benefit analysis. Compared to traditional cost-benefit analysis, we implement distributional weighting based on equivalent incomes, a new concept of individual well-being that does respect individual preferences but is not subjectively welfarist. Individual preferences are estimated on the basis of a contingent valuation question, introduced into a representative survey of the French population. Compared to traditional cost-effectiveness analysis in health technology assessment, we show that it is feasible to go beyond a narrow evaluation of health outcomes while still fully exploiting the sophistication of medical information. Sensitivity analysis illustrates the relevancy of this richer welfare framework, the importance of the distinction between an ex ante and an ex post approach, and the need to consider distributional effects in a broader institutional setting.

La loi de financement de la securite sociale (LFSS) pour 2012 a introduit le critere d’efficience dans la determination du prix des produits de sante innovants. La mise en œuvre de cette disposition par la Haute Autorite de sante (HAS) et le Comite economique des produits de sante (CEPS) met en lumiere les enjeux de l’evaluation economique pour eclairer la decision en sante. Pour documenter le critere d’efficience, la Haute Autorite de sante privilegie les analyses cout-resultat (ACR) comparant le gain en sante a son cout marginal. L’interpretation du resultat quantitatif issu de ces analyses est confrontee a la necessite de definir collectivement le montant que la collectivite est prete a depenser pour obtenir une unite de sante supplementaire. Quatre methodes permettant d’estimer cette valeur de reference sont identifiees dans la litterature. Elles permettent toutes d’estimer un indicateur d’acceptabilite, mais elles souffrent de limites qui appellent la poursuite de recherches academiques sur ce sujet. Pour autant, les evaluations produites par les industriels, l’analyse critique qui en est faite par la HAS et les avis d’efficience rendus par la Commission d’evaluation economique et de sante publique (CEESP) eclairent la relation entre le cout d’un produit de sante et les resultats de sante qu’il genere. L’elaboration recente des avis d’efficience en France semble jouer un role effectif dans la negociation des prix, meme si son usage dans la decision reste largement confidentiel. A l’image des pratiques observees dans les pays etrangers, le critere d’efficience dans les decisions en sante doit trouver sa place dans un arbitrage deliberatif et transparent parmi les autres criteres de jugement, identifies comme pertinents pour definir la valeur sociale d’un produit de sante.

Depuis le 3 octobre 2013 en France, les industriels qui revendiquent le remboursement d’un produit de sante innovant ayant, ou etant susceptible d’avoir, un impact significatif sur les depenses de l’assurance maladie ont l’obligation de fournir a la Haute Autorite de sante (HAS) une evaluation de l’efficience de leur produit. Cette evaluation est expertisee par le service d’evaluation economique et de sante publique (SEESP). A partir de cette expertise, la Commission d’evaluation economique et de sante publique (CEESP) emet un avis sur l’efficience previsible ou constatee du produit, qu’elle transmet au Comite economique des produits de sante (CEPS), charge de determiner le prix du produit de sante. En 22 mois d’activite, la HAS a rendu 30 avis d’efficience.Apres avoir rappele la place des avis d’efficience dans le circuit francais de remboursement et de fixation des prix, l’objectif de cet article est de dresser un premier bilan sur les avis d’efficience rendus par la HAS.Le processus, construit sur le principe d’un echange entre l’industriel et la HAS, a permis a la CEESP de documenter le critere economique dans la majorite des dossiers (20/30), tout en caracterisant le degre d’incertitude qui l’entoure. Pour dix dossiers, des reserves methodologiques majeures ont conduit la CEESP a rejeter la demonstration de l’industriel.

International audience; L'évaluation économique en santé au prisme de l'économie normative : principes allocatifs et règles de priorisation Résumé Depuis 2012, en France, les industries pharmaceutiques qui sollicitent l'admission au remboursement pour un traitement innovant, sont contraintes de soumettre une évaluation économique auprès de la Commission d'évaluation économique et de santé publique (CEESP) de la Haute Autorité de Santé (HAS) afin qu'un avis sur son efficience puisse être rendu. Comme dans la plupart des pays européens, le résultat de ces évaluations est mis en perspective, dans le cadre des délibérations, avec d'autres critères de décisions plus ou moins explicites, telles que la sévérité de la maladie, l'âge des patients, les situations de fin de vie, etc. L'objectif de cet article de rapporter ces critères mobilisés par les agences pour moduler l'utilisation des ratios coût/résultat à quelques grands principes normatifs de l'économie du bien-être. Cette analyse permet d'expliciter les motivations distributives du régulateur et d'évoquer les voies possibles d'amendement du calcul économique en santé pour prendre en considération les critères de priorisation mobilisés par les agences dans leurs processus de décision, tels que la priorité accordée aux patients les plus sévèrement atteints ou encore ceux dont l'espérance de vie est la plus diminuée. L'article rend ainsi compte des possibilités pratiques proposées dans la littérature. Dans quelle mesure ces approches pourraient-elles favoriser l'acceptabilité sociale des méthodes d'évaluation économique auprès des différentes parties prenantes (décideurs, communautés médicales, patients, grand public) et accroître l'utilisation des conclusions de ces évaluations dans les décisions en matière de fixation des prix et de remboursement des produits de santé ?

The aim of this article is to assess dilemma raised by adaptive preferences in the economic evaluation of growth hormone (GH) treatment for non-GH-deficient short children, and of bilateral cochlear implants for deaf children. Early implementation of both technologies and their irreversible consequences increase the potential conflicts faced by the assessors of health-related quality of life (HRQoL) states (on behalf of patients) who could be interviewed (parents, individuals with an experience of the same disability, or representative samples of the general public). Indeed, assessors' preferences may be influenced by their own situation and they are likely to vary according to age and the experience of disability. Three options are put forward which aim to resolve these moral dilemma and help economists make methodological choices that cannot be avoided in order to carry out this assessment. They are grounded on three specific egalitarian theories of social justice. The main contribution of this article is to show that a dialogue between ethics and economics, prior to an assessment, makes it possible to redefine the choice of effectiveness criteria (subjective well-being, capabilities or social outcomes), the choice of perspective (patients or the able-bodied), as well as the scope of assessment (medical and non-medical care).

L’objectif de l’article est de comparer les méthodologies d’évaluation économique de trois agences publiques chargées d’apporter une aide à la décision en matière de définition du périmètre des soins remboursables : le NICE (Angleterre et Pays-de Galles), l’IQWiG (Allemagne) et le KCE (Belgique). La grille d’analyse s’appuie sur la littérature sur les théories économiques de la justice sociale. Elle s’articule autour de deux axes : 1. Comparaison des choix sur les critères de définition des gains en santé, 2. Comparaison des choix sur les critères de répartition des gains en santé dans la population. Elle permet d’observer que les positions des agences en matière de répartition des gains en santé s’écartent de l’objectif de maximisation de la somme des satisfactions des préférences individuelles sur lequel repose le raisonnement économique de type utilitariste. Ces oppositions de principe peuvent s’expliquer par la prééminence de certaines doctrines philosophiques. Il s’agirait donc de poursuivre cette réflexion en examinant comment les outils développés en économie du bien-être permettent de prendre en compte d’autres modèles normatifs dans le cadre de l’évaluation des stratégies de santé. Our objective is to review health economic evaluation guidelines and compare methods from three national health technology assessment agencies - NICE (England and Wales), IQWiG (Germany) and KCE (Belgium) - and to assess how differences in methods may be explained by differences in philosophical views. We constructed a typology based on social choice and welfare economics theories and focussed on two main lines: 1. Definition of health gains, 2. Allocation of health gains in the population. The study showed that the guidelines of NICE, IQWiG and KCE, are moving away from the goal of maximizing the aggregation of individual utilities which comes from utilitarianism. These differences could be explained by philosophical theory influencing the definition of social contract. Welfare economics encompasses other public choice theories besides utilitarianism. Whether these economic tools could be utilized in health technology assessment is an ongoing debate.

Since 3rd October 2013 in France, drug companies applying for reimbursement of an innovative and expensive drug or medical device are required to provide the French National Authority for Health (HAS) with a cost-effectiveness assessment of their product. After a methodological audit of the economic evaluation submitted by the drug company, the Health Economics and Public Health Committee (CEESP) issues an opinion on the expected or observed cost-effectiveness. This opinion is sent to the Pricing Committee (CEPS) which determines the price of the product. After summarizing the French reimburse'!lent and pricing system, the objective of this article is to review the first 22 months of activity, in which HAS issued 30 cost-effectiveness opinions. The process, based on exchanges between drug companies and HAS, allowed the pricing committee to document the economic criterion in the majority of applications, while characterizing the degree of uncertainty of the results. For ten applications, major methodological concerns led the CEESP to reject the drug company's assessment.

The aim of this paper is to provide an assessment of individual uncertainty regarding length of life. We have collected original data through a survey performed in 2009 on a representative sample of 3,331 French people aged 18 or more. The survey design recorded several survival probabilities per individual, which makes it possible to compute (i) subjective life expectancy, defined as the first moment of the individual’s subjective distribution of personal longevity; (ii) the standard error of this distribution, which provides insight on the individual’s uncertainty regarding his or her own longevity. There is considerable between-individual variability in subjective life expectancies, in (small) part explained by age, illnesses, risky behavior, parents’ death and socioeconomic variables. The second main finding is that individual subjective uncertainty about length of life is quite large, equal on average to more than 10 years for men and women. It is logically decreasing with age, but apart from age, very few variables are correlated with it. These results have important consequences for public health and retirement policy issues.

We compare two approaches to measuring inequity in the health distribution. The first is the concentration index. The second is the calculation of the inequality in an overall measure of individual well-being, capturing both the income and health dimensions. We introduce the concept of equivalent income as a measure of well-being that respects preferences with respect to the trade-off between income and health, but is not subjectively welfarist since it does not rely on the direct measurement of happiness. Using data from a representative survey in France, we show that equivalent incomes can be measured using a contingent valuation method. We present counterfactual simulations to illustrate the different perspectives of the approaches with respect to distributive justice.
