Purpose: We hypothesize that delayed phase imaging does not provide additional diagnostic information in patients who undergo multi-phasic CTA for suspected active bleeding. Methods: Data on patients who underwent multiphasic CTA (pre-contrast, arterial, porto-venous, and delayed phases) for suspected acute bleed were retrospectively collected between January 2019 and November 2021. CTA images were reviewed by a general radiologist, an interventional radiologist, and a body imaging radiologist independently. Each reader evaluated if delayed phase images provided additional information that would change the final impression of the CTA report. Additional information regarding bleeding location, time needed for delayed image acquisition, and radiation exposure were also obtained. Results: A total of 104 patients with CTAs were analyzed with an average age of 58 years ± 22. Studies rated with absent additional findings on delayed images were 102 (98.1%) by the interventional radiologist, 101 (97.1%) by the body imaging radiologist, and 100 (96.1%) by the general radiologist with percent agreement of 96.15% (kappa 0.54, p < 0.001). All the findings were characterized as unlikely to be clinically significant. Mean time added to complete a delayed phase images was 3.61 ± 3.4 min. The average CT dose length product (DLP) for the total exam was 3621.78 ± 2129.57 mGy.cm with delayed acquisition adding a mean DLP of 847.75 ± 508.8 mGy.cm. Conclusion: Delayed phase imaging does not provide significant additional diagnostic information in evaluating patients with suspected active bleeding but is associated with increased examination time and radiation exposure. Objectives: To evaluate the effects on vascular enhancement of either a fixed rate (FR) or a fixed injection duration (FID) in single-pass (SP) contrast-enhanced abdominal multi-detector CT (CE-MDCT)., Methods: Ninety-nine (54 M; 45 F; aged 18-86 years) patients with nontraumatic acute abdomen underwent a SP CE-MDCT after i.v. injection of 1.7 cc/Kg of a nonionic iodinated contrast media (370 mgI/mL) performed with either a FR (2 cc/s; Group A) or a FID (55 s; Group B). In both groups, patients were further stratified according to total body weight (kg) as follows: 40-60 (L); 61-80 (M); 81-100 (H). Signal- (SNR) and contrast-to-noise ratios (CNR) were calculated for the liver and for both abdominal aorta (AA) and main portal vein (MPV). Statistical analysis was performed by Student t- or Chi-square test for continuous and categorical data, respectively, whereas post hoc analysis was performed by the Mann-Whitney test (P < .05)., Results: There were no significant differences in demographic and physical characteristics between Group A (n = 50; 53 ± 20 years; BMI = 23.4 ± 4.4) and Group B (n = 50; 51 ± 17 years; BMI 22.7 ± 4.2). Whereas overlapping findings were observed in the M sub-groups (n = 40), SNR and CNR were significantly higher (P < .01) in Group B for both AA and MPV in the high (H) weight sub-groups (n = 20) while not significant differences were observed in the low (L) weight sub-groups (n = 40) despite a significantly lower injection rate (1.6 ± 0.2 cc/s, P < .01) in Group B., Conclusion: A FID results in an overall better vascular enhancement than a FR in SP CE-MDCT., Advances in Knowledge: Single-pass is an optimized contrast-enhanced abdominal CT protocol combining the benefits of vascular and visceral enhancement and characterized by a customized scan delay tailored around a monophasic contrast injection. Introduction: To optimize CT chest protocol by comparing venous contrast timing with arterial timing for contrast opacification in vessels, qualitative image quality and radiologists' satisfaction and diagnostic confidence in assessing for potential nodal, pleural and pulmonary disease in general oncology outpatients. Method: Matched case−control study performed following CT protocol update. 92 patients with a range of primary malignancies with 2 CT chests in a 2‐year period, one with an arterial phase protocol and the second in the 60 second venous phase, were included. Contrast attenuation in aorta, pulmonary artery and liver were measured. Subjective measurements assessed perivenous artefact, confidence in nodal pleural and pulmonary assessment and presence of pulmonary emboli. Statistical analysis was performed using paired and unpaired t‐tests. Results: Venous‐phase CT demonstrated more consistent enhancement of the vessels, with higher attenuation of the nodes, pulmonary and pleural lesions. There was a significant reduction in perivenous beam hardening artefact on venous‐phase CT (P < 0.001). Diagnostic confidence was significantly higher for nodal assessment and pleural abnormality visibility (P < 0.001) and pleural assessment (P < 0.05). There was no significant difference in pulmonary mass visibility. There was adequate enhancement to diagnose significant pulmonary emboli (PE) with 4 incidental PEs detected on the venous phase, extending to segmental vessels. Conclusion: Venous‐phase CT chest performs better than arterial‐phase on all fronts, without compromising assessment of incidental pulmonary emboli. Abstract Background Femoral cannulation for extracorporeal circulation (ECC) is a standard procedure for minimally invasive cardiac surgery (MICS) of the atrio-ventricular valves. Vascular pathologies may cause serious complications. Preoperative computed tomography-angiography (CT-A) of the aorta, axillary and iliac arteries was implemented at our department. Methods Between July 2017 and December 2018 all MICS were retrospectively reviewed (n = 143), and divided into 3 groups. Results In patients without CT (n = 45, 31.5%) ECC was applied via femoral arteries (91.1% right, 8.9% left). Vascular related complications (dissection, stroke, coronary and visceral ischemia, related in-hospital death) occurred in 3 patients (6.7%). In patients with non-contrast CT (n = 35, 24.5%) only femoral cannulation was applied (94.3% right) with complications in 4 patients (11.4%). CT-angiography (n = 63, 44.1%) identified 12 patients (19.0%) with vulnerable plaques, 7 patients (11.1%) with kinking of iliac vessels, 41 patients (65.1%) with multiple calcified plaques and 5 patients (7.9%) with small femoral artery diameter (d ≤ 6 mm). In 7 patients (11.1%) pathologic findings led to alternative cannulation via right axillary artery, additional 4 patients (6.3%) were cannulated via left femoral artery. Only 2 patients (3.2%) suffered from complications. Conclusions CT-A identifies vascular pathologies otherwise undetectable in routine preoperative preparation. A standardized imaging protocol may help to customize the operative strategy.

PurposeThe purpose of this study was to determine the frequency in which the pelvis component of an abdominopelvic CT provides information that would influence clinical management in two separate groups of patients: those with previously resected pancreatic ductal adenocarcinoma (PDA) and those with locally advanced unresectable PDA.MethodsThis institutional review-board approved HIPAA compliant retrospective study with waived informed consent included 247 subjects with histologically proven PDA, including 153 subjects post-pancreaticoduodenectomy and 94 subjects with locally advanced unresectable disease. Imaging reports interpreted between January 2005 and December 2013 were obtained from our institution's Radiology Information System by searching a Cancer Registry database of PDA patients separately for the words "whipple" and "unresectable." CT findings were separated by location in the abdomen or pelvis, and subsequently reviewed and graded for their likelihood of representing metastatic disease. The probability of pelvic CT influencing clinical management-i.e., of finding isolated pelvic metastatic disease-was determined using 95% binomial proportion confidence intervals for both the post-pancreaticoduodenectomy and locally advanced unresectable groups.ResultsNo subjects who had undergone pancreaticoduodenectomy had an isolated pelvic metastasis on follow-up imaging (0%; 95% CI 0-2.38, p = 0.0004); 33 had metastatic disease in the abdomen, and 120 had no or equivocal evidence of abdominopelvic metastatic disease. One subject with locally advanced unresectable PDA had a possible isolated pelvic metastasis on follow-up imaging (1.1%; 95% CI 0.03-5.79, p = 0.048); 20 had metastatic disease in the abdomen, and 73 had no or equivocal evidence of abdominopelvic metastatic disease.ConclusionIsolated pelvic metastatic disease rarely occurs in patients with PDA who have had prior pancreaticoduodenectomy or have a locally advanced unresectable primary tumor, suggesting routine pelvic CT in follow-up imaging of these patients may not be necessary.

Abstract Background Coronary CT angiography (CCTA) is a complicated CT exam in comparison to other CT protocols. Exam success highly depends on image assessment of experienced radiologist and the procedure is often time-consuming. This study aims to evaluate feasibility of automatic CCTA reconstruction in 0.25 s rotation time, 16 cm coverage CT scanner with best phase selection and AI-assisted motion correction. Methods CCTA exams of 90 patients with heart rates higher than 75 bpm were included in this study. Two image series were reconstructed—one at automatically selected phase and another with additional motion correction. All reconstructions were performed without manual interaction of radiologist. A four-point Likert scale rating system was used to evaluate the image quality of coronary artery segment by two experienced radiologists, according to the 18-segment model. Analysis was done on per-segment basis. Results Total 1194 out of the 1620 segments were identified for quality evaluation in 90 patients. After automatic best phase selection, 1172 segments (98.3%) were rated as having diagnostic image quality (scores 2–4) and the average score is 3.64 ± 0.55. When motion corrections were applied, diagnostic segment number increases to 1192 (99.8%) and the average score is 3.85 ± 0.37. Conclusions With the help of 0.25 s rotation speed, 16-cm z-coverage and AI-assisted motion correction algorithm, CCTA exam reconstruction could be performed with minimum radiologist involvement and still meet image quality requirement.

In recent years, Photon-counting detector CT (PCD CT) has emerged as a new and groundbreaking technology in clinical radiology. While clinical research and practical applications of PCD-CT are constantly evolving, it has not yet been integrated into post-mortem CT (PMCT) imaging. Documented research into the potential applications of PCD CT in the field of post-mortem human forensic pathology and anatomical pathology is scarce in literature. This is despite the fact that PCD CT shows promise in expanding PMCT imaging diagnostic due to features such as ultra-high resolution and intrinsic spectral multienergy imaging. The authors have recently started scanning forensic corpses with a PCD CT to explore its possibilities and limits in the post-mortem field. Because of availability and low radiation dose level, low dose computed tomography (CT) is now commonly used to identify illicit in corpore drug transportation. This review illustrates the most common CT findings of in corpore drug transportation and describes complications due to in corpore drug transportation, with a special emphasis on low dose CT. Major information such as number of packets, exact location and aspect of packets must be assessed. Radiologist must be aware of the imaging characteristics of " in corpore " illicit drug transportation, and should know situations that may alter drug smugglers management. • Low dose CT is commonly used to identify in corpore drug transportation. • While drug packets are mainly hyperdense on CT, they can sometimes have a lower density. • Sagittal and coronal reconstructions, as well as three-dimensional rendered views are often helpful to detect hyperattenuating drug packets • If too many packets are located in the stomach, the patient should be transferred to intensive care unit for surveillance because of a high risk of rupture. Objectives: To investigate the role of third-generation iterative reconstruction (ADMIRE) in dual-source, high-pitch chest CT protocol with spectral shaping at 100 kVp in Coronavirus disease 2019 (COVID-19). Methods: Confirmed COVID-19 inpatients undergoing to unenhanced chest CT were scanned with a dual-energy acquisition (DECT, 90/150Sn kV) and a dual-source, high-pitch acquisition with tin-filtered 100 kVp (LDCT). On the DECT with ADMIRE 3 (DECT3) were evaluated the pulmonary findings and their extension (25-point score). Two radiologists in consensus evaluated with 5-point scales the overall image quality, the anatomical structures, and the elementary findings on LDCT reconstructed with filtered backprojection (LDCT0), with ADMIRE 3 (LDCT3) and 5 (LDCT5), and on DECT3. The signal-to-noise ratio (SNR), the body mass index, the exposure times, and the radiation doses were recorded. Results: Seventy-five patients (57 M/18F; median age: 63 y.o.) were included, with median pulmonary extension of 13/25 points. The imaging findings were detected in proportion comparable to the available literature. The ADMIRE significantly improved the SNR in LDCT (p < 0.00001) with almost no significant differences in overweight patients. The LDCT had median effective dose of 0.39 mSv and acquisition time of 0.71 s with significantly less motion artifacts than DECT (p < 0.00001). The DECT3 and LDCT3 provided the best image quality and depiction of pulmonary anatomy and imaging findings, with significant differences among all the series (p < 0.00001). Conclusion: The LDCT with spectral shaping and ADMIRE3 provided acceptable image quality in the evaluation of patients with COVID-19, with significantly reduced radiation dose and motion artifacts. Patients suspected of having an acute aortic syndrome in the ED typically undergo CT of the chest/abdomen/pelvis. However, the overwhelming majority of these exams are negative. With the help of clinical decision support, we implemented a new radiologist monitored 'aortic dissection screening protocol' that forgoes routine abdominopelvic imaging in order to reduce radiation dose without compromising diagnostic accuracy. The purpose of the present study is to assess the performance of this protocol. A retrospective analysis was performed to study the effect of the dissection screening protocol on the diagnostic yield, radiation and contrast dose on a total of 835 ED patients who underwent CT scans for suspected aortic dissection over a 48-week study period immediately before and after implementation of the protocol. 3.4% (28/835) of examinations were positive for an acute aortic syndrome over the 48-week study period with no difference in positivity before and after implementation of the 'aortic dissection screening' protocol, 3.0% vs. 3.7%, respectively (p = 0.57). There was a 14.6% reduction in median radiation dose and a 16% decrease in contrast volume utilization for the total ED population who underwent CT for aortic dissection using any protocol in the period after implementation of the 'aortic dissection screening' protocol. Aortic dissection CT in the ED is negative in the overwhelming majority of cases. A monitored 'aortic dissection screening' protocol that initially images the chest only significantly reduced contrast and radiation dose without reducing diagnostic accuracy for ED patients who underwent CT for aortic dissection. • The majority of CT exams in patients imaged for acute aortic syndrome are negative. • With a monitored 'aortic dissection screening' protocol there is a reduction in contrast and radiation dose. • Using clinical decision support to guide the ordering clinician in choosing the best imaging test has a high clinical impact • The implementation of an 'aortic dissection screening' provides all the imaging necessary for TEVAR planning. • CT PNS has become a well-established preoperative diagnostic tool providing a mandatory 'roadmap' for FESS. • The phantom study was performed first, in order to determine protocols to be investigated in the patient study. • A standard CT PNS protocol can be replaced with ultra-low dose protocol. To assess image quality and radiation dose associated with ultra-low dose CT protocol for patients with benign paranasal sinus diseases undergoing functional endoscopic surgery (FESS). We scanned the head portion of Alderson RANDO phantom on a second generation, dual-source, multidetector-row CT scanner (Siemens Definition Flash) using standard-dose and five low-dose protocols. Two radiologists assessed the image quality for each protocol to determine best ultra-low-dose protocols for imaging patients with benign paranasal sinus diseases undergoing FESS. The ultra-low-dose CT protocols were then used for scanning. Thereafter, 40 adult patients (age range 18–54 years, M:F 23:17) were scanned with the four low dose scanning protocols (10 patients per protocol). On both transverse and coronal reformatted CT images, two radiologists assessed visibility of key anatomic landmarks for FESS on a 2-point scale (1 = clear and complete visualization; 2 = suboptimal visualization). Data were analyzed with descriptive statistics and Cohen's kappa coefficient for interobserver agreement. In phantom study, the lowest dose scan protocol (CTDI vol 2.1 mGy, 70 kV, 75 mAs) was unacceptable due to poor image quality. For patient studies, both radiologists gave acceptable image quality scores for ultra-low-dose scan protocol with axial scan mode, automatic tube potential selection and tube current modulation (CTDI vol 2.2 mGy; DLP 22.9 mGy.cm) with up to 60% lower dose compared to prior standard-dose CT (CTDI vol 5.3 mGy; DLP 73.5 mGy.cm). Purpose: In the literature, no consensus exists about which CT protocol is to be adopted in patients who underwent high-energy blunt trauma. The aim of the study is to evaluate the additional value of the arterial phase in the CT assessment of vascular injuries of the liver.Methods: Admission CT examinations for patients with traumatic injury of the liver due to high-energy blunt trauma, performed between 2011 and 2017 in two major trauma centres, were retrospectively reviewed. Images were analysed for presence or absence of liver parenchymal injury, intrahepatic contained vascular injuries and active bleeding in the arterial and portal venous phase of the CT study.Results: Two hundred twelve patients have been identified. Parenchymal injuries were detected as isolated in 90.6% of cases, whereas they were associated with vascular injuries in 9.4% of cases: contained vascular injuries in 3.3% and active bleeding in 6.1%. Out of all parenchymal injuries detected on the CT portal venous phase, 90.5% were also detectable in the arterial phases (p < 0.0001). All of the contained vascular injuries were visible in the CT arterial phase, whereas they were detectable in 28.5% of cases also during the venous phase (p = 0.02). All 13 cases of active bleeding were detected on the CT venous phase, and 76.9% of these cases were also revealed in the arterial phase, thus confirming their arterial origin (p = 0.22).Conclusion: The addiction of the arterial phase to the venous phase in the CT assessment of patients who underwent high-energy blunt trauma allows an accurate identification and characterization of traumatic vascular injuries, so distinguishing between patients suitable for conservative management and those requiring interventional or surgical treatment. Objective: To investigate the image quality and radiation dose of dual-energy computed tomography (DECT) with automatic spectral imaging protocol selection (ASIS) compared with those of low-kVp CT in abdominal multiphase CT.Methods: Four groups of 60 patients each underwent abdominal scans with low-kVp CT (A, 80 kVp/300 mg I/kg, body mass index [BMI] ≤ 23.9 kg/m2; C, 100 kVp/400 mg I/kg, BMI ranging from 24 to 28.9 kg/m2) or DECT with ASIS, and the 40- to 60-keV virtual monochromatic images (VMIs) generated (B and D) were matched by age, gender, BMI, cross-sectional area, and contrast agent dose; 9 patients were excluded due to technical failures. The CT number, image noise, contrast-to-noise ratio, and subjective image quality were compared between the matched protocols (A and B or C and D) on 1.25-mm reconstructed images.Results: VMIs at approximately 55 keV and 62 keV had CT numbers and contrast similar to those of 80-kVp and 100-kVp CT images, respectively. Compared to matched low-kVp images, VMIs at 50 keV provided a higher CT number and image noise and a similar or higher contrast and overall image quality. The radiation dose for DECT was higher than that of 80-kVp CT (increased by 10%), but was similar to that of 100-kVp CT.Conclusion: Compared to matched low-kVp CT, VMIs at 50 keV in DECT with ASIS provided similar or higher overall image quality, with no or minimal dose penalty in small- and medium-sized patients.Key Points: • Virtual monochromatic images at approximately 55 keV and 62 keV have CT numbers and contrast similar to those of 80-kVp and 100-kVp CT images, respectively, with a given noise index. • The radiation dose in dual-energy CT with automatic spectral imaging protocol selection was slightly higher than that of 80-kVp CT (increased by 10%) but was similar to that of 100-kVp CT. • Dual-energy CT may be able to replace l00-kVp CT for routine clinical abdominal contrast-enhanced CT scans. Aim To evaluate possibilities of computed tomography (CT) perfusion in differentiation of solitary focal liver lesions based on their characteristic vascularization through perfusion parameters analysis. Methods Prospective study was conducted on 50 patients in the period 2009-2012. Patients were divided in two groups: benign and malignant lesions. The following CT perfusion parameters were analyzed: blood flow (BF), blood volume (BV), mean transit time (MTT), capillary permeability surface area product (PS), hepatic arterial fraction (HAF), and impulse residual function (IRF). During the study another perfusion parameter was analyzed: hepatic perfusion index (HPI). All patients were examined on Multidetector 64-slice CT machine (GE) with application of perfusion protocol for liver with i.v. administration of contrast agent. Results In both groups an increase of vascularization and arterial blood flow was noticed, but there was no significant statistical difference between any of 6 analyzed parameters. Hepatic perfusion index values were increased in all lesions in comparison with normal liver parenchyma. Conclusion Computed tomography perfusion in our study did not allow differentiation of benign and malignant liver lesions based on analysis of functional perfusion parameters. Hepatic perfusion index should be investigated in further studies as a parameter for detection of possible presence of micro-metastases in visually homogeneous liver in cases with no lesions found during standard CT protocol

ABSTRACT: In a multi‐center patient study, using different CT scanners, CT‐based finite element (FE) models are utilized to calculate failure loads of femora with metastases. Previous studies showed that using different CT scanners can result in different outcomes. This study aims to quantify the effects of (i) different CT scanners; (ii) different CT protocols with variations in slice thickness, field of view (FOV), and reconstruction kernel; and (iii) air between calibration phantom and patient, on Hounsfield Units (HU), bone mineral density (BMD), and FE failure load. Six cadaveric femora were scanned on four CT scanners. Scans were made with multiple CT protocols and with or without an air gap between the body model and calibration phantom. HU and calibrated BMD were determined in cortical and trabecular regions of interest. Non‐linear isotropic FE models were constructed to calculate failure load. Mean differences between CT scanners varied up to 7% in cortical HU, 6% in trabecular HU, 6% in cortical BMD, 12% in trabecular BMD, and 17% in failure load. Changes in slice thickness and FOV had little effect (≤4%), while reconstruction kernels had a larger effect on HU (16%), BMD (17%), and failure load (9%). Air between the body model and calibration phantom slightly decreased the HU, BMD, and failure loads (≤8%). In conclusion, this study showed that quantitative analysis of CT images acquired with different CT scanners, and particularly reconstruction kernels, can induce relatively large differences in HU, BMD, and failure loads. Objective We instituted a new, simple CT pulmonary angiography (CTPA) contrast material timing protocol using a standard empiric delay to replace our previous timing bolus method. This study tests the hypothesis that the empiric protocol more consistently produces diagnostic quality images of both the pulmonary arteries and the aorta with lower radiation exposure. Materials and methods We performed a retrospective review of consecutive CTPAs for 2 months both before and after the protocol change. Pulmonary artery and aortic enhancement, patient characteristics, radiation exposure and results of CTPA were analyzed. Results There were 182 patients in the timing bolus group and 164 in the empiric timing group. Both groups had a female majority (59%) and a similar mean age (58 and 57 years, respectively). Enhancement was significantly higher both for the pulmonary artery (median 400 HU versus 359 HU, P < 0.001) and aorta (median 381 HU versus 218 HU, P < 0.01) in the empiric timing group versus the timing bolus group, respectively. Radiation exposure was lower (5.3 mSv versus 6.0 mSv, P = 0.05) in the empiric timing group, despite a higher body-mass-index (31 versus 29 kg/m 2 , P < 0.01). Pulmonary embolism positivity rate was non-significantly higher in the timing bolus vs the empiric timing group (19% and 13%, P = 0.1). Conclusion A simple empiric timing protocol for CTPA has robust performance compared to a timing bolus protocol. Empiric timing preserves the required high diagnostic quality for evaluation of the pulmonary arteries with the added benefits of aortic enhancement and lower radiation exposure. Purpose: Computed tomography (CT) examinations, often using high-radiation dosages, are increasingly used in the acute management of polytrauma patients. This study compares a low-dose polytrauma multi-phase whole-body CT (WBCT) protocol on a latest generation of 16-cm detector 258-slice multi-detector CT (MDCT) scanner with advanced dose reduction techniques to a single-phase polytrauma WBCT protocol on a 64-slice MDCT scanner.Methods: Between March and September 2015, 109 polytrauma patients (group A) underwent acute WBCT with a low-dose multi-phase WBCT protocol on a 258-slice MDCT whereas 110 polytrauma patients (group B) underwent single-phase trauma CT on a 64-slice MDCT. The diagnostic accuracy to trauma-related injuries, radiation dose, quantitative and semiquantitative image quality parameters, subjective image quality scorings, and workflow time parameters were compared.Results: In group A, statistically significantly more arterial injuries (p = 0.04) and arterial dissections (p = 0.002) were detected. In group A, the mean (±SD) dose length product value was 1681 ± 183 mGy*cm and markedly lower when compared to group B (p < 0.001). The SDs of the mean Houndsfield unit values of the brain, liver, and abdominal aorta were lower in group A (p < 0.001). Mean signal-to-noise ratios (SNRs) for the brain, liver, and abdominal aorta were significantly higher in group A (p < 0.001). Group A had significantly higher image quality scores for all analyzed anatomical locations (p < 0.02). However, the mean time from patient registration until completion of examination was significantly longer for group A (p < 0.001).Conclusions: The low-dose multi-phase CT protocol improves diagnostic accuracy and image quality at markedly reduced radiation. However, due to technical complexities and surplus electronic data provided by the newer low-dose technique, examination time increases, which reduces workflow in acute emergency situations. ABSTRACT The current study compared the impact of low radiation CT protocols on the accuracy, repeatability, and inter- and intra-observer variability of implant migration studies in total hip arthroplasty. Two total hip replacements were performed in two human cadavers and six tantalum beads were inserted into the femur similar to radiostereometric analysis. Six different 28 mm heads (−3 mm, 0 mm, 2.5 mm, 5.0 mm, 7.5 mm, and 10 mm) were added to simulate five reproducible translations (maximum total point migration) of the center of the head. Three CT scans with varying levels of radiation were performed for each head position. The effective dose (mSv) was 3.8 mSv for Protocol A (standard protocol), 0.7 mSv for Protocol B and 1.6 mSv for Protocol C. Implant migration was measured in a 3-D analysis software (Geomagic Studio 7). The accuracy was 0.16 mm for CT Protocol A, 0.13 mm for Protocol B and 0.14 mm for Protocol C; The repeatability was 0.22 mm for CT Protocol A, 0.18 mm for Protocol B and 0.20 mm for Protocol C; ICC for inter observer reliability was 0.89, intra observer reliability was 0.95. The difference in accuracy between standard protocol A and the two low radiation protocols (B, C) was less than 0.05 mm. The accuracy, inter- and intra-observer reliability of all three CT protocols is comparable to radiostereometric analysis. Reducing the CT radiation exposure to numbers similar to an AP Pelvis radiograph (0.7 mSv protocol B) does not affect the accuracy of implant migration measurements. Purpose To evaluate the utility and radiation dose of thoraco-abdominopelvic precontrast CT in polytrauma patients. Materials and methods We examined retrospectively 125 patients who underwent a thoraco-abdominopelvic CT for trauma. Two radiologists, independently, evaluated precontrast CT acquisition and two other radiologists examined the contrast-enhanced scans. A further two radiologists assessed both the acquisitions. Mean value of sensitivity (SE), specificity (SP), positive predictive value (PPV) and negative predictive value (NPV) were calculated by each group of radiologists. For 104 patients, CTDIvol, DLP data and individual mean size were collected to calculate effective dose. Results Mean values of SE, SP, PPV and NPV of findings of radiologists who assessed contrast-enhanced acquisitions were respectively: SE = 85%, SP = 98%, PPV = 86%, NPV = 88% versus: SE = 43%, SP = 95%, PPV = 69%, NPV = 88% of radiologists who examined non-contrast-enhanced scans. Mean values of radiologists who analyzed both acquisitions were: SE = 80%, SP = 97%, PPV = 80%, NPV = 88%. Neither the precontrast scans nor the precontrast and postcontrast scans together provided additional useful information compared to the single contrast-enhanced acquisition. Patients received a mean dose of 12 mSv for the precontrast CT. Conclusions Precontrast CT acquisition did not provide significant information in trauma patients, exposing them to an unjustified radiation dose. [ABSTRACT FROM AUTHOR]

With growing concern over radiation exposure from CT, dose reduction and optimization have become important considerations. Many protocol factors and CT technologies influence this dose reduction effort, and as such, users should maintain a working knowledge of developments in the field. Individual patient factors and scanner-specific details also require care and expertise, which are vital to the success of any dose reduction effort. The authors review the content of the Virtual Symposium on Radiation Safety in Computed Tomography (University of California Dose Optimization and Standardization Endeavor), specifically that pertaining to the more practical aspects of dose optimization. These range from prescan tips to postscan factors, as well as protocol definition itself. Topics discussed include localizer radiograph acquisition, tube current modulation, reconstruction methods, and pediatric considerations, with the content biased toward a CT technologist or protocol manager. Near-term innovations, including new iterative reconstruction methods, tube potential modulation, and dual-energy CT, are presented, and their capability for dose reduction is briefly discussed. The aim of this study was to compare the radiation dose and image quality of different adenosine-stress dynamic myocardial perfusion CT protocols using a 128-slice dual-source computed tomography (DSCT) scanner. We included 330 consecutive patients with suspected coronary artery disease. Protocols employed the following dynamic scan parameters: protocol I, a 30-s scan with a fixed tube current (FTC, n = 172); protocol II, a 30-s scan using an automatic tube current modulation (ATCM) technique (n = 108); protocol III, a 14-s scan using an ATCM (n = 50). To determine the scan interval for protocol III, we analyzed time-attenuation curves of 26 patients with myocardial perfusion who had been scanned using protocol I or II. The maximum attenuation difference between normal and abnormal myocardium occurred at 18.0 s to 30.3 s after initiation of contrast injection. Myocardial perfusion images of FTC and ATCM were of diagnostic image quality based on visual analysis. The mean radiation dose associated with protocols I, II, and III was 12.1 ± 1.6 mSv, 7.7 ± 2.5 mSv, and 3.8 ± 1.3 mSv, respectively ( p < 0.01). Use of a dose-modulation technique and a 14-s scan duration for adenosine-stress CT enables significant dose reduction while maintaining diagnostic image quality. [ABSTRACT FROM AUTHOR]
