A terapia do Par Biomagnético (PB) visa tratar a causa das doenças através do uso das duas polaridades de ímãs de média intensidade colocados sobre o corpo, em pontos específicos, com a finalidade de detectar e corrigir desequilíbrios eletromagnéticos e distorções do potencial hidrogeniônico. Neste relato de caso, foi aplicada a terapia do PB em uma criança diagnosticada com meningite tuberculosa (MTB) que se encontrava em tratamento convencional há quase cinco meses. O paciente queixava-se de dor lancinante e apresentava outros sinais e sintomas neurológicos. Após duas sessões de terapia do PB, foi possível eliminar a dor e constatar melhora no quadro geral.

Objective To compare the techniques for the osteosynthesis of intertrochanteric fractures with dynamic hip screws (DHSs) through the Hardinge and minimally-invasive access routes of the hip, evaluating the operative time, the degree of pain in the immediate postoperative period, the hematimetric loss, and the functional aspects of active mobility. Methods A randomized, double-blinded clinical trial in which 66 patients with intertrochanteric fractures were submitted to osteosynthesis by DHS. The patients were divided into a test group, submitted to the minimally-invasive access, and a control group, in whom the surgery was performed through the Hardinge route. Results Patients submitted to the minimally-invasive treatment presented a lower degree of postoperative pain compared to the group treated by the Hardinge lateral route (p< 0.001), as well as lower hematimetric loss (p< 0.001), shorter operative time (p< 0.001), and improvement in immediate postoperative active mobility tests (p

Numa proposta de atenção integral à saúde do paciente, empreendeu-se uma revisão narrativa que buscou compreender se os pacientes hospitalizados que recebem assistência espiritual/religiosa apresentam melhores resultados em saúde. Como objetivo geral deste artigo, pretendeu-se ressaltar a importância da dimensão espiritual/religiosa na vida de pacientes hospitalizados. Realizou-se uma busca por literatura em três grandes bases de dados eletrônicas: Biblioteca Virtual em Saúde (BVS), PubMed e o Portal de Periódicos da CAPES. O número de material encontrado, após a busca inicial nas três bases de dados, foi igual a 292 artigos (Portal de Periódicos da CAPES = 238 artigos; BVS = 44 artigos; PubMed = 10 artigos). Selecionaram-se 10 artigos que atenderam aos critérios de inclusão. Participaram desta revisão 4.121 pacientes, aproximadamente, de ambos os sexos e com diversos diagnósticos (em situação de internação). Os trabalhos selecionados demonstraram que o indivíduo em situação de hospitalização, muitas vezes, encontra-se sem saber como agir diante de tal contexto e tende a fortalecer-se através da busca pelo sagrado, pelo divino, daquilo que pode dar novo sentido para a sua vida e para a sua morte, que se percebe mais próxima que antes. Dessa forma, o hospital aparece como um ambiente onde as incertezas humanas surgem, sejam de ordem física, psicossocial e/ou espiritual, sendo necessário um cuidado que atenda a todas essas dimensões da subjetividade humana. In a proposal of integral attention to the health of the patient, a narrative review was undertaken that sought to understand if hospitalized patients who receive spiritual/religious assistance present better health outcomes. As a general objective of this article, it was intended to emphasize the importance of the spiritual/religious dimension in the life of hospitalized patients. A literature search was carried out in three large electronic databases: Biblioteca Virtual em Saúde (BVS), PubMed and Portal de Periódicos da CAPES. The number of material found, after the initial search in the three databases, was equal to 292 articles (Portal de Periódicos da CAPES = 238 articles, BVS = 44 articles, PubMed = 10 articles). We selected 10 articles that met the inclusion criteria for participation. A total of 4,121 patients participated in this review, approximately of both sexes and with several diagnoses (hospitalized). The selected works showed that the hospitalized individual often finds himself unsure how to act in such a context and tends to be strengthened through the search for the sacred, for the divine, of what can give new meaning to his life and for his death, which is perceived closer than before. In this way, the hospital appears as an environment where human uncertainties arise, whether of a physical, psychosocial and / or spiritual nature, and a care is required that meets all these dimensions of human subjectivity.

Fundação de Amparo a Pesquisa do Estado de Minas Gerais The herbivory is an important factor for the structuring and diversity of natural communities, and may be influenced by factors such as seasonality, and spatial interactions by plant-herbivores-predators. In response to herbivory plants have different defense strategies, mainly chemical, physical, phenological or biotic. The associations with ants, attracted by extrafloral nectaries (EFN), are probably the main biotic defenses of plants. This thesis aims to determine the variation over time, the benefits conferred by the ant EFN visitors from Ouratea spectabilis (Ochnaceae) in the Cerrado environment, against the action of herbivores if all species of ants actually protect the plant to receive extrafloral nectar and how the ants win by visiting extrafloral nectaries. We set up three experiments in isolation: one in which we manipulated in the laboratory, ants and plants with and without EFNS to measure the gain that the ants get in the extrafloral nectar as a food supplement. In the second chapter, we evaluated the performance of a species of ant (Cephalotes pusilus ) to see if all the species actually confer protection to plants with EFN and a final experiment, by the time spent and the exclusion of ants, we evaluated the interaction simultaneously for two years in Uberlândia - MG, Água Clara - MS and Brasilia - DF, in order to test the geographical mosaic theory of coevolution. We demonstrated for the first time, in an obvious way, that ants benefit by feeding on extrafloral nectar, increasing the weight and number of individuals in the course of a year. We showed that some ants, in this case Cephalotes pusilus do not confer protection to the plant and that just feed on nectar and pollen, showing that not all species behave aggressively and have features to protect the plant. In the final experiment, we could demonstrate that the interactions between ants and Ouratea spectabilis have the same ecological significance regardless of geographic region. It is suggested that one type of interaction is the result of coevolution, the set of interacting elements are subject to similar selective pressures that lead to interaction for the same outcome in this case, a mutualistic interaction, which corroborates the theory that was tested A herbivoria é um importante fator de estruturação e diversidade das comunidades naturais, e varia em função de sazonalidade, variação espacial e pelas interações plantaherbívoros- predadores. Como resposta à herbivoria, as plantas apresentam diferentes estratégias de defesa, principalmente químicas, físicas, fenológicas ou bióticas. As associações com formigas, atraídas por nectários extraflorais (NEFs), são provavelmente as principais defesas bióticas dos vegetais. A presente tese como objetivo avaliar a variação ao longo do tempo dos benefícios conferidos pelas formigas visitantes de NEFs de Ouratea spectabilis (Ochnaceae) no ambiente Cerrado, contra a ação de herbívoros. A hipótese que todas as espécies de formigas protegem a planta ao receber néctar extrafloral. Para tal, avaliou-se o quanto as formigas ganham ao visitar nectários extraflorais. Montamos três experimentos, isoladamente: no primeiro, manipulamos em laboratório, formigueiros e plantas com e sem NEFS para mensuramos o ganho das formigas em receber o néctar extrafloral foi testada como alimento complementar; no segundo avaliamos em campo o desempenho de uma espécie de formiga (Cephalotes pusilus) para saber se todas as espécies realmente conferem proteção as plantas portadoras de NEFs e em um ultimo experimento, através da permanência e exclusão de formigas, avaliamos a interação simultaneamente por dois anos em Uberlândia MG, Água Clara MS e Brasília DF, afim de testar a Teoria dos Mosaicos Geográficos da Coevolução. Conseguimos demonstrar pela primeira vez, de forma evidente, que formigas obtém benefícios ao se alimentar de néctar extrafloral, aumentando o peso e número de indivíduos no decorrer de um ano. Conseguimos demonstrar que algumas formigas, neste caso Cephalotes pusilus, não confere proteção alguma a planta e que simplesmente se alimenta de néctar e pólen, mostrando que nem todas as espécies se comportam de maneira agressiva e possuem características para proteger a planta. E no ultimo caso, conseguimos demonstrar que as interações entre formigas e Ouratea spectabilis apresentam o mesmo significado ecológico, independendo da região geográfica. Sugere-se que se um tipo de interação é resultado de coevolução, o conjunto de elementos interagentes está sujeito a pressões seletivas semelhantes que conduzirão a interação para um mesmo resultado, neste caso, uma relação mutualística, corroborando com a teoria que foi testada. CAPES - Coordenação de Aperfeiçoamento de Pessoal de Nível Superior Durante os últimos anos o enfoque nas interações entre formigas e plantas tem recebido grande atenção, principalmente em regiões tropicais. Este tipo de interação tem fornecido informações para um melhor entendimento do mutualismo, de caráter facultativo e do processo coevolutivo com várias espécies de angiospermas, principalmente as que possuem nectários extraflorais (NEFs) que oferecem o néctar como alimento as formigas. No presente estudo foi investigado o aspecto da condicionalidade nos resultados das relações ecológicas entre formigas - plantas no Cerrado, ou seja, como a presença ou ausência de formigas afeta a ação dos herbívoros de uma árvore do ao longo do tempo e verificar se a ocorrência de fogo pode afetar as relações entre formigas e plantas no Cerrado. O presente estudo foi desenvolvido na reserva do Clube de Caça e Pesca Itororó de Uberlândia (CCPIU), Uberlândia — Minas Gerais (18°59'S, 48°18'W) de setembro de 2004 á janeiro de 2005 em uma área não queimada e outra queimada. A espécie com NEFs foi Ouratea spectabilis, Engl. (Ochnaceae) que é uma das espécies mais abundantes em todos os cerrados. O estudo foi dividido em quatro etapas:1) Marcação das plantas e montagem da manipulação experimental (exclusão das formigas com adição de uma resina — Tanglefoot®); 2) Mensuração da herbivoria foliar; 3) Impacto reprodutivo e identificação de insetos herbívoros, 4) Identificação de formigas associadas. Foram encontradas 35 espécies de formigas diurnas, noturnas e uma, Camponotus rufipes, que visitou os NEFs em ambos os períodos do dia. As formigas visitantes de O. spectabilis se utilizaram tanto dos NEFs quanto do exsudato de hemípteros (Membracidae) que podem infestar as panículas das infrutescências. Foram observados herbívoros pertencentes a 10 famílias, com destaque a coleópteros, lepidópteros e tisanópteros. Os resultados dos experimentos avaliando o efeito do fogo e da presença ou ausência da ação da interação formiga-planta ao longo do tempo sobre a herbivoria, mostraram que a presença de formigas visitantes dos NEFs de O. spectabilis reduziu significativamente a herbivoria foliar. A visitação de formigas também foi relevante para a aptidão da planta, indivíduos com formigas formaram mais frutos por botões produzidos, tanto na área queimada, quanto na área não queimada. A quantidade de sementes dos frutos produzidos com entre áreas não queimadas e queimadas com a presença de formiga também foi maior. Os benefícios da associação com formigas para O. spectabilis se mantém ao longo do tempo. A interação parece ser igualmente vantajosa e importante para formigas e plantas nos dois ambientes (com ou sem o fogo), dependendo da presença ou não de formigas. During the last years the focus in the interactions between ants and plants has been receiving great attention, mainly in tropical areas. This interaction type has been supplying information for a better understanding of the optional mutualism and of the coevolution process with several angiosperms species, mainly the ones that possess extrafloral nectaries (EFNs) that offer the nectar as food to the ants. In the present study the aspect of the conditionality was investigated in the results of the ecological relationships among ants - plants in the Cerrado, in other words, how the presence or absence of ants affects the action of the herbivores of a tree through time and to verify how the fire occurrence can affect the relationships between ants and plants in the Cerrado. The present study was developed in the reservation of Clube de Caya e Pesca Itororo de Uberlandia (CCPIU), Uberlandia - Minas Gerais (18°59'S, 48°18'W) of September of 2004 to January of 2005. The areas were separeted as burned and not burned. The species with ENFs were Ouratea spectabilis, Engl. (Ochnaceae), this is one of the most abundant species in all the Tropical Savanna. The study was divided in four stages:1) Demarcation of the plants and assembly of the experimental manipulation (exclusion of the ants with addition of a resin - Tanglefoot®); 2) Measurement of the leaf herbivory 3) Reproductive impact and identification of herbivore insects, 4) Identification of associated ants. 36 morfoespecies of day ants and night ants that were found Camponotus rufipes, that visited NEFs in both periods of the day. The visiting ants, 0. spectabilis used EFNs, as well as honeydew of hemiptera (Membracidae) that can infest the panicules of the infrutescences. We observed 10 herbivore's families, with prominence to coleopterons, lepidopterous and tisanopters. The results of the experiments evaluating the effect of the fire and the presence or absence of the action of the interaction ant-plant along the time on the herbivory, they showed that the ants visitor's of EFNs of 0. spectabilis presence reduced the herbivoria significantly to foliate. The visitation of ants was also relevant for the aptitude of the plant, individuals with ants formed more fruits for produced buds in the burned area as well as, the not burned area. The amount of seeds of the fruits produced among both areas with the ant presence were also larger. The benefits of the association with ants for 0. spectabilis stay along the time. The medical residency has an excellent concept of medical training, but it suffers several criticisms in the resident physician's quality of life requirement, due to a sudden change in their lifestyle and a work rate increased by the workload required by the residency legislation doctor. Objective: The main objective of this study was to analyze the Quality of Life, prevalence of Burnout Syndrome and Daytime Sleepiness in physicians from various medical specialties: Internal Medicine, General Surgery, Anesthesiology, Otorhinolaryngology, Orthopedics and Gynecology and Obstetrics in a reference hospital in Manaus. Propose the structuring of the Support Center for Resident Physicians and Preceptors (NAMP) with the objective of implementing programs to improve the quality of life of resident physicians, research and training for preceptors. Method: The following questionnaires were given: 1. World Health Organization Quality of Life Assessment (WHOQOL); 2. Epworth Sleepiness Scale (ESS) for daytime sleepiness assessment; and 3. Maslach Burnout Inventory (MBI). According to Maslach???s criteria, Burnout was studied in its dimensions: emotional exhaustion, depersonalization, and personal fulfillment. Results: It was identified that a Burnout Syndrome has a prevalence of 77.42% in the studied residents. As for the self-assessment of quality of life, using the WHOQOL ??? abbreviated WHO questionnaire, we obtained an average of 5.42, which may indicate that there are factors in the residency that may be affecting the quality of life of these physicians. Regarding the levels of sleepiness in the daytime, a value of 13.21 was found for the group of resident physicians studied. Conclusion: The results obtained with the Burnout frames are related to exposure to stressors of working conditions. Therefore, a preventive approach to those factors that have a negative impact on residents??? health is necessary. In this sense, the structuring of the NAMP is proposed. M??todo: Foram aplicados os seguintes question??rios: 1. Avalia????o da qualidade de vida (WHOQOL) da Organiza????o Mundial de Sa??de (OMS); 2. Escala de Sonol??ncia Epworth (ESE) para avalia????o de sonol??ncia diurna; e 3. Maslach Burnout Inventory (MBI). De acordo com os crit??rios de Maslach, o Burnout foi estudado em suas dimens??es: exaust??o emocional, despersonaliza????o e realiza????o pessoal. Resultados: Foi identificado que a S??ndrome de Burnout teve a preval??ncia de 77,42% nos m??dicos residentes estudados. Conclus??o: Os resultados sugerem que os quadros de Burnout est??o relacionados ?? exposi????o aos fatores estressores das condi????es de trabalho. Logo, a abordagem preventiva nesses fatores que exercem impacto negativo sobre a sa??de dos m??dicos residentes torna-se necess??ria. Nesse sentido, ?? proposta a estrutura????o do NAMP.

Viral reservoirs are the main barrier to achieve a cure for HIV in infected individuals, even in the presence of Antiretroviral Therapy (ART). HIV infected macrophages migrate to various tissues, forming a stable viral reservoir in continuous production. TREM-1 is a myeloid cell protein-1, expressed in monocytes/macrophages and neutrophils, fundamental in the regulation and amplification of inflammation in the immune response. Objective: To assess whether the sTREM-1 protein has a predictive value for mortality in HIV/AIDS patients hospitalized for HIV-related causes of hospital admission, in a well-known and reputed hospital in the city of Manaus-AM, Brazil. Methodology: A prospective cohort study was carried out in patients hospitalized at the Funda????o de Medicina Tropical Doutor Heitor Vieira Dourado (FMT ??? HVD), with a 28-day segment for predicting mortality. A total of 340 patients who met the eligibility criteria were identified in the study, in which 260 were included. sTREM-1 plasma was quantified by ELISA. To evaluate the correlation between sTREM-1 and mortality prediction in HIV patients, the ROC Curve, Area Under the Curve (AUC) and Poisson Multivariate Regression were calculated. Results: From 260 patients included, 81.9% survived and 18.1% died. The study of domains contributes to the analysis of interventions in prostate cancer especially when HRQol is in divergence to survival gain. Objective: To investigate the health-related quality of life of patients with prostate cancer in radiotherapy alone (VMAT) or adjuvence in a care unit in the city of Manaus. Method: This is a cross-sectional study with a quantitative approach with the application of a socioeconomic/clinical questionnaire and the European Organization for Researchand Treatment of Cancer Qualityof Life Questionnaire "Core" 30 items (EORTC-QLQ-C30). The sample studied consisted of 21 men with prostate cancer in radiotherapy who were predominantly elderly in tumor stage II. One third of these patients observed underwent prostatectomy prior to radiotherapy. Results: In the clinical survey, hypertension and sexual dysfunction were the most frequent conditions. When assessing HRQoL, social function presented better results and emotional function revealed the highest negative impact with lower mean score. In the symptom scale, the most affected domain was constipation with higher mean and median. There was an association with statistical significance between the intervals of radiotherapy doses and the scale of loss of appetite, especially in high doses (1100 cGy ??? 12000 cGy). The loss of appetite domain also showed a positive correlation with the age variable. Conclusion: The identification of critical health conditions and quality of life domains such as sexual dysfunction, social function, constipation and loss of appetite reinforce these aspects as objects of etiological studies and therapeutic effectiveness. In addition, they assist clinical practice by increasing evidence by promoting therapeutic decision-making as well as interventions to control paraeffects. The desire for a quick, effective and painless treatment, led to the search for skin substitutes for the lesions that generated the loss of protection given by its natural barrier to aggressive agents, with delayed healing and prolonged suffering, causing increased tissue damage. and pain. Man has already sought the ideal ointment on land, at sea and even in the air, researching the benefit of some gases. Technological advancement with the appearance of the nanoscale has brought us back to electrospinning, allowing us to re-evaluate the application, a centuries-old process of making materials, with widespread use in industry, combined with new substances for research in the health area. OBJECTIVE: General: Evaluation of the applicability of poly (?? - caprolactone) membranes loaded with drugs in the treatment of wounds. Specifics: Check which types of wounds HS and AGE loaded membranes and pure PCL membranes are applicable to. Compare effectiveness between drug loaded membranes and pure PCL membranes. Estimate the cost-benefit ratio in the application of the membranes. METHOD: observational, prospective, randomized, and controlled study with the participation of patients with clean or potentially contaminated wounds in patients admitted to the Hospital e Pronto Socorro 28 de Agosto, in the city of Manaus - Reference hospital in Amazonas. Patients were randomly assigned to three groups: Group 1 - 8 patients treated with pure membrane. Group 2 - 8 patients treated with membrane releasing sodium heparin, Group 3 - 8 patients treated with membrane Essential Fatty Acids. The results were obtained by comparing the groups, with treatment by Friedman's nonparametric method. RESULTS: There were statistically significant differences in the characteristics of malleability, adhesion, transparency, with superiority of the pure membrane in relation to the others. The low absorption capacity of all membranes leads to the indication for covering non-secreting lesions. All membranes allowed an important decrease in the pain of the treated patients and there was no infection, confirming the effectiveness of the ozone sterilization method. CONCLUSIONS: all membranes used in this research have good applicability with an indication for lesions that affect up to the deep dermis, and there is a need to make macropores to evaluate the possibility of application in higher grade lesions. The low cost of production was found and, finally, the great savings generated with the application of this material were observed, highlighting the significant importance of continuing this research for the benefit of the population. JUSTIFICATIVA: A cobertura ideal das feridas tem sido um desafio para a humanidade desde os prim??rdios. O anseio por um tratamento r??pido, efetivo e indolor, levou a busca de substitutos de pele para as les??es que geravam a perda da prote????o dada por sua barreira natural aos agentes agressores, com retardo na cicatriza????o e prolongamento de sofrimento, provocando aumento de danos teciduais e da dor. O homem j?? buscou o unguento ideal na terra, no mar e at?? no ar, pesquisando o benef??cio de alguns gases. O avan??o tecnol??gico com o aparecimento da escala nanom??trica, nos trouxe de volta a eletrofia????o, permitindo que reavali??ssemos a aplica????o um processo secular de confec????o de materiais, com o uso difundido na ind??stria, combinado com novas subst??ncias para pesquisas na ??rea da sa??de. OBJETIVOS: Geral: Avalia????o da aplicabilidade de membranas de poli (?? ??? caprolactona) carregadas com f??rmacos no tratamento de feridas. Espec??ficos: Verificar a que tipos de feridas as membranas carregadas com HS e AGE e das membranas de PCL puras s??o aplic??veis. Comparar a efic??cia entre as membranas carregadas com f??rmacos e membranas de PCL puras. Estimar a rela????o custo-benef??cio na aplica????o das membranas. M??TODO: estudo observacional, prospectivo, randomizado e controlado com a participa????o de pacientes portadores de feridas limpas ou potencialmente contaminadas em pacientes internados no Hospital e Pronto Socorro 28 de Agosto, na cidade de Manaus ??? Hospital de refer??ncia no Amazonas. Os pacientes foram distribu??dos aleatoriamente em tr??s grupos: Grupo 1 ??? 8 pacientes tratados com a membrana pura. RESULTADOS: Observou-se diferen??as estatisticamente signifativas nas caracter??sticas de maleabilidade, ader??ncia, transpar??ncia, com superioridade da membrana pura em rela????o as demais. A baixa capacidade de absor????o das todas a membranas leva a indica????o para a cobertura de les??es n??o secretoras. Todas as membranas possibilitaram importante diminui????o no quadro ??lgico dos pacientes tratados e n??o houve ocorr??ncia de infec????o confirmando a efic??cia do m??todo de esteriliza????o com oz??nio. CONCLUS??ES: todas as membranas utilizadas neste estudo possuem boa aplicabilidade com indica????o para les??es que acometam at?? a derme profunda, e h?? necessidade de confec????o de macroporos para avaliar a possibilidade de aplic????o em les??es de maior grau. Among patients with atheromatous disease are those with lesions in one or multiple coronary vessels, especially those considered to be at high risk, indicated to undergo coronary artery bypass graft surgery. Atheromatous disease is complex, of chronic evolution and even those undergoing coronary artery bypass surgery, need to remain in treatment of this disease for the rest of their lives, but many leave the hospital without understanding the need to maintain specialized treatment, guided by a Multidisciplinary Team. General objective: To prepare a Manual with multidisciplinary guidelines for patients in the postoperative period of coronary artery bypass graft surgery at Francisca Mendes University Hospital in the city of Manaus-Amazonas. Specific objectives: Investigate scientific literature on the subject of myocardial revascularization, interact with a Multidisciplinary Team on strategies for post-operative myocardial revascularization guidelines and organize the selected content for the elaboration of a Manual. Method: This is a qualitative study of health research with the design thinking methodology and the use of the Brainstorming tool. Expected results: To contribute to the reduction of complications in the postoperative period of coronary artery bypass surgery with care guidelines prepared by a Multidisciplinary Team that seek to remove fear and insecurity, so that the patient rescues physical and mental well-being, improving quality of life and safe return to work activities. Reduction of hospital costs for preventable hospitalizations and promote an increase in the degree of understanding about the disease and greater adherence to outpatient follow-up. Examination requests are routinely observed, which most often present results within normal limits and do not influence preoperative management. In this sense, it is common to find difficulties in following a protocol in the preoperative evaluation, since, in general, each professional draws up his own flowchart, causing a series of obstacles to the performance of surgical procedures. Objective (general): To develop a preoperative evaluation protocol for patients referred to elective surgery at the General Surgery Service of Hospital Universit??rio Get??lio Vargas (HUGV). Objectives (specific): 1. Conduct a situational analysis of the preoperative evaluation of elective general surgery developed at HUGV; 2. Promote discussion among the participating professionals to collect indications for the elaboration of the protocol; 3. Build a protocol for use in the preoperative evaluation based on the indications of the participating professionals, articulating their foundation with the scientific literature; 4. Develop a flowchart for the follow-up of the preoperative evaluation. Methods: This is a qualitative study using the action research methodology, using strategic planning tools such as the Likert Scale, SWOT Matrix and Brainstorming in order to emerge and substantiate the data. Such instruments helped in the survey of factual situations, positive and negative, in the development of this study in order to present proposals and solutions in the construction of the protocol. After data exploration, they were organized and analyzed through content analysis. Results: In the discussion of the data, primary categories emerged, such as: university hospital, multidisciplinary and regional aspects, which underlie the construction of criteria for preoperative evaluation and requests for complementary exams. In line with the professional indications, the elaboration of the protocol is structured based on the following elements: age, associated comorbidities, findings on physical examination and surgical size. The analyzed categories supported the construction of the criteria in the elaboration of the protocol. As a final result, in addition to the elaboration of the protocol, the importance of collaborative work is emphasized. The articulation of the multidisciplinary team in the construction of the work, based on the exchange of practices and knowledge, provided the experience with richness and ensured the validity of the flowcharts and the protocol as a final product. Justificativa: O paciente com indica????o de uma cirurgia eletiva, ao contr??rio de uma cirurgia de emerg??ncia, pode ser estratificado e modo a diminuir o tempo de espera, bem como prevenir eventos adversos. Observa-se, de forma rotineira, a solicita????o de exames, que na maioria das vezes apresentam resultados dentro dos padr??es de normalidade e n??o influenciam no manejo pr??-operat??rio. Nesse sentido, ?? frequente encontrar dificuldades no seguimento de um protocolo na avalia????o pr??-operat??ria, pois, em geral cada profissional elabora seu pr??prio fluxograma, acarretando uma s??rie de entraves para a realiza????o dos procedimentos cir??rgicos. Objetivo (geral): Elaborar um protocolo de avalia????o pr??-operat??ria para pacientes encaminhados a cirurgia eletiva no Servi??o de Cirurgia Geral do Hospital Universit??rio Get??lio Vargas (HUGV). Objetivos (espec??ficos): 1. Realizar an??lise situacional da avalia????o pr??-operat??ria de cirurgia geral eletiva desenvolvida no HUGV; 2. Promover discuss??o entre os profissionais participantes para coleta de indica????es para elabora????o do protocolo; 3. Construir um protocolo para a utiliza????o na avalia????o pr??-operat??ria a partir das indica????es dos profissionais participantes articulando sua fundamenta????o com a literatura cient??fica; 4. Elaborar um fluxograma para o seguimento da avalia????o pr??-operat??ria. M??todos: Trata-se de um estudo qualitativo com emprego da metodologia pesquisa-a????o, utilizando-se ferramentas de planejamento estrat??gico como a Escala Likert, Matriz SWOT e Brainstorming com o intuito de emergir e consubstanciar os dados. Tais instrumentos auxiliaram no levantamento de situa????es f??ticas, positivas e negativas, no desenvolvimento desse estudo com o intuito de apresentar propostas e solu????es na constru????o do protocolo. Ap??s explora????o dos dados, os mesmos foram organizados e analisados atrav??s de an??lise de conte??do. Resultados: Na discuss??o dos dados emergiram categorias prim??rias, como: hospital universit??rio, multidisciplinaridade e aspectos regionais, os quais fundamentam a constru????o de crit??rios para avalia????o pr??-operat??ria e solicita????es de exames complementares. Em conson??ncia com as indica????es profissionais a elabora????o do protocolo se estrutura a partir dos seguintes elementos: idade, comorbidades associadas, achados no exame f??sico e porte cir??rgico. As categorias analisadas fundamentaram a constru????o dos crit??rios na elabora????o do protocolo. Como resultado final, al??m da elabora????o do protocolo, ressalta-se a import??ncia do trabalho colaborativo. A articula????o da equipe multidisciplinar na constru????o do trabalho, a partir da troca de fazeres e saberes, oportunizou riqueza ?? experi??ncia e assegurou a validade dos fluxogramas e do protocolo como produto final.

The survey was attended by 57 professionals from two different hospitals. The instruments used for data collection were: sociodemographic questionnaire, structured interview, Duke Religious Index (DUREL), Intrinsic Religiousness Inventory (IRI), WHOQOL-SRPB, Coping With Death Scale and Collett-Lester's Fear of Death Scale. The sample consisted of 83.9% of female participants, with an average age of 37.31 years (SD = 9.72). The intrinsic religiousness index found was high (M = 46.61; SD = 5.96), and whether or not religion had no effect on intrinsic religiousness. Regarding the WHOQOL-SRPB, the average found was 16.52 points (SD = 2.04) and there was no average difference between people with and without religion. The professionals of the hospital teams had a moderate fear of the death of others (M = 3.26; SD = 1.16) and a neutral ability to cope with death (M = 93.36; SD = 24.55). Qualitative analyzes have demonstrated dual feelings regarding the death of patients and the influence of religious thinking on conceptions of death. It was concluded that religiousness constitutes an important factor for the quality of life of the professionals surveyed and provides meaning and comfort in the face of the death of patients, but in isolation it is not able to minimize the fear of death. O presente estudo teve por objetivo investigar se os profissionais de equipes de sa??de hospitalares utilizam a sua religiosidade para melhor lidar com a morte e o morrer de pacientes. A pesquisa contou com a participa????o de 57 profissionais de dois hospitais distintos. Os instrumentos utilizados para a coleta de dados foram: question??rio sociodemogr??fico, entrevista estruturada, Escala de Religiosidade Duke (DUREL), Invent??rio de Religiosidade Intr??nseca (IRI), WHOQOL-SRPB, Escala de Coping Frente ?? Morte e Escala de Medo da Morte de Collett-Lester (EMMCL). A amostra se constituiu por 83,9% de participantes do g??nero feminino, com idade m??dia de 37,31 anos (DP = 9,72). O ??ndice de religiosidade intr??nseca encontrado foi alto (M = 46,61; DP = 5,96), e ter ou n??o religi??o n??o teve efeito sobre a religiosidade intr??nseca. Em rela????o ao WHOQOL-SRPB, a m??dia encontrada foi 16,52 pontos (DP = 2,04) e n??o houve diferen??a m??dia entre pessoas com e sem religi??o. If on one hand, the difficulty in obtaining this exam has a direct impact on the efficiency of public health and delays diagnosis and initiation of treatment, on the other hand, professionals face great difficulties in establishing such Services due to a lack of organizational knowledge, either owing to numerous regulations required by inspection agencies or to a lack of perception of the needs of the environment. There is no book available on the topic, only one-off, ???loose??? research materials on the internet. Then, why not compile those materials into a practical book that allows for quick and easy consultation for the organization of an Endoscopy Service and that is of interest to all the actors involved in such health practice scenario, allowing for improvements in efficiency, effectiveness and impact on the provision of services? Objective: Creation of a technical book with guidelines for multidisciplinary teams providing Endoscopy Services. Methods: Literature review with research on papers from the following databases: Virtual Health Library (VHL), MEDLINE, SCIELO and LILACS, searching for these descriptors: ???Endoscopy???, ???Digestive???, ???Service???, in addition to searches in specialized books, endoscopy medical societies, and in government, deontological and legal websites. Results/Conclusions: The book was written and peer reviewed by the permanent faculty of the HUGV-EBSERH-UFAM and advised by the professors of the Post- Graduate Program in Surgery ??? PPGRACI ??? Thus, skin healing is a fundamental element for human health, both in the occurrence of injuries and in surgical procedures. The effectiveness of the healing process can be further enhanced through the development of products that originate from plants with traditional use, this importance is enhanced when we consider that p Brazil is a megadiverse country, being the Amazon region the great granary of this diversity. Thus, in this study we sought to develop a healing compound based on andiroba (Carapa guianensis), copa??ba (Copaifera langsdorffii) and Guarana (Paullinia Cupana) oils, three plants of Amazonian origin that have potential healing effect via phase modulation inflammatory and proliferative fibroblasts. It is possible that the combination of oils and guarana will result in a potential innovative pharmacological product with high pro-healing potential. OBJECTIVES: General: Develop and analyze the effect of a product obtained via combined extraction of andiroba, copaiba and guarana (ACG) in the form of biphasic oil and emulsion through the analysis of in vitro and in vivo healing models. Specific: Perform the analysis using dermal fibroblasts and Californian worm Eisenia fetida, in vivo model of body regeneration via surgical incision of the caudal region. METHOD: Because it is a study that does not involve direct contact with vertebrates, there is no need for approval by the Ethics Committee. After obtaining the ACG the antioxidant and genotoxic / genoprotective capacity was evaluated in non-cellular tests. For the tests using the commercially derived dermal fibroblast line HFF-1 and Californian worms (Eisenia Fetida), also commercially obtained. The cells were cultured under standard conditions, and in these strach assay (in vitro skin lesion model) tests were performed. The cells were treated with ACG at different concentrations and after 24/72 hour inclination, viability tests / cell proliferation. Growth markers, oxidative stress and inflammation were analyzed. As a result, tremor develops at rest, muscle stiffness and bradykinesia. Although PD also causes non-motor symptoms related to the main causes of death in these individuals, scientific research mainly addresses neuromotor symptoms. Objective: To analyze the risk of dysphagia, respiratory muscle strength (FMR) and maximal phonation time (MPT) in elderly patients with Parkinson's disease in the initial stages. Method: an observational study in the elderly with PD in the initial stages (Hoen & Yahr modified 0-3), enrolled in the Comprehensive Attention Centers of the Best Age - CAIMIS Dr. Paulo Lima and Dr. Andr?? Ara??jo - in Manaus, Amazonas, Brazil. We evaluated the risk of dysphagia with the Eating Assessment Tool (EAT-10) questionnaire; FMR by manovacuometry and vocal behavior by TMF (sustained fricatives / s / and / z / e of its s / z ratio). The absolute and relative absolute frequencies for the categorical data were presented. For the quantitative data, when the normality hypothesis was accepted using the Shapiro-Wilk test, the mean and the standard deviation were calculated. When the normality hypothesis was rejected, the median and the interquartile range (IIQ) were calculated. In the comparison of means in relation to categorical variables, the t-student test was applied. The Mann-Whitney test was used to compare the medians. In the analysis of the quantitative data the Pearson's Correlation Coefficient and the t-student test were calculated to verify if the Coefficient was nonzero. The software used was Minitab?? version 17 for Windows??. The level of significance was 5%. Results: In the elderly with PD in the early stages there was a higher risk of dysphagia, lower FMR and association of respiratory and phonological components. FMR worsened within 3 years of follow-up, and as PEm??x decreased, the risk of dysphagia increased. Conclusion: In the elderly with PD, there was alteration of respiratory and phonatory variables, even at the beginning of the disease, demonstrating that the evaluation of these variables should be a propaedeutic and, if altered, therapeutic target, not awaiting respiratory, swallowing and / or phonatory symptoms. It is a disorder of the non-contagious and nonfatal movement caused by the progressive loss of the ventrolateral cells of the compact part of the black substance of the midbrain, which leads to three cardinal signs: rest tremor, stiffness and bradykinesia. Although the scientific research mainly addresses neuromotor signs, there are other functional damages, and the consequences of ventilatory dysfunction are the main cause of death in this population. Objective: To analyze pulmonary function and respiratory muscle strength (FMR) of elderly patients with PD at an early stage. Method: An observational study evaluating the degree of dysfunction related to the disease through the modified Hoehn & Yahr scale, FMR by manovacuometry, and pulmonary volumes and capacities by spirometry. FMR and lung function were evaluated in the on and off period of levodopa (L-Dopa). The results were presented through graphs and tables, with simple absolute frequencies and relative to the categorical data. For the analysis of the quantitative data, when the normality hypothesis was accepted using the Shapiro-Wilk test, the mean and the standard deviation (SD) were calculated. When the normality hypothesis was rejected, it was decided to calculate the median and the interquartile range (IIQ). In the comparison of means, the t-student test was applied, and in the case of the dependent variables the paired t-student test. In the comparison of medians, the Mann-Whitney test for the independent variables was applied, and Wilcoxon for dependent variables. The Minitab version 17 program was used for Windows and the level of significance in the statistical tests was 5%. Results: Elderly patients with early-stage PD had a higher prevalence of ventilatory disorder than their controls, mainly the restrictive disorder. There was no respiratory muscle weakness and no difference between the respiratory muscle strength of the elderly with PD in relation to their controls, matched by gender, age and body mass index. The on-off period of L-Dopa didn???t change the analyzed variables. Conclusion: Elderly patients with PD present ventilatory disorders, even in the early stages of the disease, being the restrictive disorder the most prevalent. L-Dopa does not change the analyzed variables. Resultados: Os idosos com DP em est??gio inicial da doen??a apresentaram maior preval??ncia de dist??rbio ventilat??rio que seus controles, principalmente o dist??rbio restritivo. N??o houve fraqueza muscular respirat??ria e nem diferen??a entre a FMR dos idosos com DP em rela????o a seus controles, pareados por sexo, idade e ??ndice de massa corporal. However, few epidemiological studies exist on the clinical profile of newborns (NBs) born in the north of Brazil.Objectives: Characterize the clinical profile of NBs in a Neonatal Intensive Care Unit (NICU) of high complexity, in a reference maternity hospital to risk pregnancy in Manaus/Amazonas. Methods: It was an observational, cross-sectional, descriptive, analytical, retrospective study involving NBs hospitalized between the periods of 2014 to 2016 in a NICU of high complexity in a Public Maternity of reference to high-risk pregnancy of the city of Manaus ??? Amazonas. The study followed all the required criteria involving research with human beings, being approved by the Ethics Committee in Research (CAAE: 82751217.3.0000.5613, parecer: 2.556.757). To analyze the quantitative data, when accepted the hypothesis of normality through the Shapiro- Wilk test, the average and the standard deviation (SD) were calculated. The deleterious effect of malnutrition associated with metabolic changes that occur in the perioperative period may increase the risk of complications such as infection, metabolic and hemodynamic disorders, increased hospital stay, increased hospital costs, reduced quality of life, and patient survival. One way to prevent or reduce these effects is to identify patients early and initiate appropriate nutritional management, but the medical decisions regarding nutritional behavior are still insipient, due to several factors. Thus, tools that can help in a practical way in the search for knowledge and decision making benefit not only the professional but also the patient. OBJECTIVE: To prepare a book for quick consultation for the assistance of health team professionals in the care of the surgical patient in nutritional therapy. Specifics objectives: To investigate in the scientific literature recommendations on nutritional therapy in surgical patients; analyze and select the information found in the scientific literature; organize the content for writing the book. METHOD: This is a methodological study that includes an integrative review, with a documentary and retrospective analysis that covers the five years of scientific publication. It was used as search sites: Scientific Electronic Library Online - Scielo, Public Medline - PubMed, Web of Science, COCHRANE Liberty, and the database of relevant scientific societies. RESULTS: We found 836 articles of which 90 were selected. The articles were compiled by topic addressed to the definition of content resulting in 11 chapters of the book. For the preparation of the chapters, new information pertinent to the subject has been integrated. CONCLUSION: It was possible to elaborate the book, containing the themes compiled from the analysis and selection of the articles found in the review. O efeito delet??rio da desnutri????o associada as altera????es metab??licas que ocorrem no per??odo perioperat??rio pode aumentar os riscos de complica????es como infec????o, dist??rbios metab??licos e hemodin??mico, aumento do tempo de interna????o, aumento do custo hospitalar, redu????o da qualidade de vida e sobrevida do paciente. In Brazil, a total of 59,700 new cases is estimated for 2018, being the main oncological cause of female death in the country. The expression of estrogen receptors (ERs) and progesterone receptors (PRs) and overexpression of human epidermal growth factor receptor-2 (HER2) have been identified as important predictive factors among breast cancer patients, being currently used to define the treatment and establish the prognosis of the disease. GOALS: The purpose of the present analysis was to identify the frequency of immunohistochemical subtypes of invasive breast cancers treated at Funda????o Centro de Controle de Oncologia do Estado do Amazonas (FCECON), which are registered in the pathology laboratory of the institution, and to reach its purpose, specific objectives were set: i) to identify the frequency of hormone-receptor positivity in carcinomas-in-situ (CIS); ii) to identify the frequency of the histopathological subtypes of the case series; iii) to relate the age and gender of patients in the case series according to the histopathological and immunohistochemical status of the lesion; and; iv) suggest strategies that facilitate the dissemination and understanding of the meaning of the results of immunohistochemistry for breast cancer patients using research data. METHOD: For this, an observational, descriptive and retrospective study was conducted with breast cancer diagnosed patients and also those with an immunohistochemistry medical report in the files of the FCECON pathology laboratory, in Manaus, Amazonas, during a 9-year period. Data was obtained from secondary data sources and later included in an EXCEL database and transferred to STATA?? Version 13 where the statistical calculations were performed. RESULTS: It had been studied 1604 cases of patients diagnosed with breast cancer treated at the FCECON from 2008 to 2016, averaging 178.2 cases per year. Of these cases, 1597 (99.6%) were female and 7 (0.4%) were male. In the female cases, the age ranged from 25 to 95 years (average 52.6 years). The percentage of women under 40 years was 14.8%, within 40 years to 65 years was 68.2% and more than 65 years was 17%. The most frequent histopathological type was invasive ductal carcinoma (IDC) with 89.9%. The most common subtype was Luminal B (LB) with 32.6% of cases, followed by Negative Triple (TN) 24.3%. It is an ideally surgical teatment condition which makes this, a disease associed associated with serious morbidity. The technique fixation must be reproducible, less agressive, with low medical complication rates and good functional results. The Dynamic Hip Screw (DHS) system is one of several implants options used worldwide for the treatment of these fractures, and its installation through MIPO approach (Minimally Invasive Plate Osteosynthesis) is poorly documented in world literature. Objectives: The objective of this study is to compare the fixation techniques of intertrochanteric fractures with DHS, using the Hardinge approach and a minimally invasive approach, while assessing pain levels, hematimetric loss, inflammatory alterations, as well as evaluating functional aspects in the immediate post-operative follow-up. Method: This is a randomized, double-blind clinical trial in which 70 patients underwent osteosynthesis by the DHS system. The patients were divided into a test group, in which the approach used was MIPO and one control, with the Hardinge approach. The MIPO???s group showed a lower degree of postoperative pain, compared to the group treated with the Hardinge approach. Coordenação de Aperfeiçoamento de Pessoal de Nível Superior In Brazil, economic growth causes rapid increases in marine transports. The impact of this increase on marine mammals, combined with the lack of legal enforcement, is potentially dangerous. Knowledge of vessel-dolphin interaction is essential to Brazilian marine ecology. The study of dolphin behavior can identify problems and support actions for conservation. Our principle aim was to study vessel-dolphin interactions and determine whether group and infant behaviors were being affected. Observations (600hs) were conducted in Pontal do Paraná and Guaraqueçaba (Southern Brazil), in 2010. We used a mixed method of \"focal-group sampling\" and \"sequential sampling\" for behavior records. Boats were classified according to engine type: central (diesel) and outboard (petrol). Interactions were classified into positive, neutral and negative based on observed behaviors before, during and after vessel passage (947 events were recorded). The dolphins behavioral responses varied according to the engine type, speed and distance. Results showed increased frequency of neutral interactions when the observed distance between boats and dolphins was greater than 100m with vessel at low speed (0.1 to 2.0 km/h). The neutral and positive interactions were more frequent with center engine boats, probably because these vessels do not reach high speed (faster than 5km/h). All vessel types changed the dolphin s behaviors in some way. The distance between boats and dolphins influences the animal s surface behavior when interaction occurred with central motor vessels in Guaraqueçaba (z =- 5105, p= 0.000) and with outboard boats in Pontal (z = -3682, p =0.000) and Guaraqueçaba (z =- 3931, p = 0.000). The variable speed influenced surface behavior when interaction occurred with outboard boats in Guaraqueçaba (z = 2,107, p = 0.035). Changes in group structure, after vessel passage, such as mother and calve separation were observed, especially when the boat split the group. These data will contribute to human activities management in the region. No Brasil, o crescimento econômico provocou um rápido aumento no tráfego de embarcações. O crescimento desse impacto sobre os mamíferos marinhos somado a falta de legislação específica é potencialmente perigoso. Conhecer as interações entre embarcações e golfinhos é essencial para ecologia marinha brasileira. O estudo do comportamento deste grupo pode identificar problemas e subsidiar ações para a conservação. O objetivo deste estudo foi analisar interações entre o botos e embarcações e se estas alteram os comportamentos dos grupos e infantes. As observações (600hrs) foram realizadas em Pontal do Paraná e Guaraqueçaba (Sul do Brasil), em 2010. Para registro dos comportamentos foi utilizado um método misto de \"grupo focal\" e \"amostragem sequencial\". As embarcações foram separadas de acordo com o tipo do motor: centro (diesel) e popa (gasolina). As interações foram divididas em: positiva, neutra e negativa, baseadas nos comportamentos observados antes, durante e depois da passagem das embarcações (947 interações observadas). A resposta comportamental dos golfinhos observados variaram em relação ao tipo de motor, velocidade e distância entre os indivíduos e embarcações. Os resultados mostraram um aumento na frequência de interações neutras quando a distância observada entre os barcos e golfinhos foi maior que 100m com embarcações em velocidade baixa (0,1-2,0 km/h). As interações positivas e neutras foram mais frequentes com embarcações de motor de centro, provavelmente porque estas embarcações não atingem velocidade alta (maior que 5km/h). Todos os tipos de embarcações alteraram de alguma forma as atividades que vinham sendo desenvolvidas pelos botos. A distância entre barcos e golfinhos influenciou o comportamento de superfície do boto-cinza quando a interação ocorreu com embarcações de motor de centro em Guaraqueçaba (z = - 5,105, p = 0,000) e com os barcos de motor de popa em Pontal (z = -3.682, p = 0,000) e Guaraqueçaba (z = - 3,931, p = 0,000). A variável velocidade influenciou o comportamento de superfície quando a interação ocorreu com barcos de popa em Guaraqueçaba (z = 2.107, p = 0,035). Mudanças na estrutura, após a passagem da embarcação, tais como separação de mãe e filhote, também foram observadas, principalmente quando a embarcação passava sobre o grupo. 44. The role of spirituality in the well-being of families with children with congenital heart disease: scoping review.

- Abstract
Background: Congenital heart disease (CHD) is the leading cause of death from malformations in infants and has a significant psychological impact on families. This scoping review explored the role of spirituality in supporting families of children with CHD. It also sought to identify gaps in the existing literature and suggest directions for future research., Methods: A synthesis review was conducted following the framework of Arksey and O'Malley, incorporating the principles of the Joanna Briggs Institute. We systematically searched four databases-Medline, Embase, Lilacs, and Scielo-selecting studies based on inclusion criteria focused on spirituality in families of children with CHD. Articles without full text, book chapters, lectures, conference abstracts, review articles, and editorials were excluded. We analyzed the year of publication, study location, objectives, methodology, participants of the study, and main results., Results: A total of 17 studies were included in the review, most of which were conducted in the past 10 years. The studies were conducted in various regions of the world, including Japan, the USA, China, Iran, Sweden, and Brazil, and they examined different religious traditions, including Christianity, Islam, Buddhism, and non-denominational spiritual practices. Twelve qualitative studies, three descriptive studies, one cross-sectional study, and one case report were included. The articles were categorized based on several themes: the influence of spirituality on mental and emotional well-being, cultural and religious diversity, integration of spirituality into health care, and its assistance in decision-making., Conclusions: Spirituality plays a crucial role in addressing the emotional and psychological challenges of CHD. There is a need for structured models of spiritual counseling and more research in diverse cultural and socioeconomic contexts. 45. Fixation of Intertrochanteric Fractures with Dynamic Hip Screws: Randomized Controlled Trial Comparing Hardinge and Minimally Invasive Approaches.

- Abstract
Objective To compare the techniques for the osteosynthesis of intertrochanteric fractures with dynamic hip screws (DHSs) through the Hardinge and minimally-invasive access routes of the hip, evaluating the operative time, the degree of pain in the immediate postoperative period, the hematimetric loss, and the functional aspects of active mobility. Methods A randomized, double-blinded clinical trial in which 66 patients with intertrochanteric fractures were submitted to osteosynthesis by DHS. The patients were divided into a test group, submitted to the minimally-invasive access, and a control group, in whom the surgery was performed through the Hardinge route. Results Patients submitted to the minimally-invasive treatment presented a lower degree of postoperative pain compared to the group treated by the Hardinge lateral route ( p < 0.001), as well as lower hematimetric loss ( p < 0.001), shorter operative time ( p < 0.001), and improvement in immediate postoperative active mobility tests ( p <0.05). Conclusion The study demonstrated the clinical superiority of the minimally-invasive access route parameters analyzed in relation to the Hardinge access for the fixation of intertrochanteric fractures when DHS is the choice osteosynthesis method. - Abstract
The management of patients, such as Jehovah's Witnesses, who refuse to receive blood transfusions, is often a medical challenge, not only because of the ethical dilemma, but also because it creates a major obstacle to rapid hemorrhage control in a setting of trauma. This article explores the reasons for this conflict between the physician's duty of care and the respect for the patient's autonomy, and draws a panorama of the main understandings of the Judiciary on the subject. Finally, it is concluded that the manifestation of the patient's will, although free, is not enough to release the doctor from his (her) duty of care. In case of danger to life, the doctor must carry out a blood transfusion, regardless of the patient's consent or the permission of those responsible for the patient.
