1. Subclinical hepatitis C virus infection in Egyptian patients with rheumatic diseases: a multi-center study

Abstract: Abstract Background Hepatitis C virus (HCV) infection is highly prevalent in Egypt. It was found to be 7.0% in a study done in 2015. There are some studies on the prevalence of HCV in rheumatoid arthritis, but to our knowledge, no previous study was done to detect it in other rheumatologic diseases. This study aims at detecting the prevalence of subclinical HCV infection in Egyptian patients with different rheumatic diseases. In the current study, eight hundred and three patients with different rheumatic diseases collected from five—geographically different—Egyptian rheumatology departments were studied. Patients with known current or previous HCV infection were excluded from the study. Screening for the positive anti-HCV antibodies was done for all patients. Screening for the presence of HCV ribonucleic acid (RNA) was done in patients with positive serology by reverse transcriptase-polymerase chain reaction. Results In the studied population, 675 (84.1%) patients were women. The mean age [± SD] was 44.2 [± 12.9] years. Hepatitis C antibody positivity was found in 73 (9.1%) of the patients, while 67 (8.3%) were having positive HCV-RNA quantitative PCR tests. The highest prevalence of seropositive HCV was found in drug-induced vasculitis (DIV) and cryo-vasculitis (100%), while in RA, HCV antibodies and PCR were found to be positive in 9.1% and 8.3% of patients, respectively. Conclusions Detection of the presence of HCV infection in 9.1 % of the studied middle-aged Egyptian patients with rheumatologic conditions points to the importance of screening for HCV in such population for early detection and intervention especially for those patients that are planned to start biologic therapy.

3. The remote revolution: assessing the impact of working from home on finance professionals

Abstract: Abstract The aim of this study is to investigate how remote work affects job performance. It suggests that factors such as the work environment, employee productivity, and work-life balance play a role in mediating this impact. The study also explores how remote work influences employee performance through these variables. The findings of this research have practical implications for businesses looking to evaluate the effectiveness of their remote work policies, especially during the ongoing coronavirus crisis. One distinctive feature of this study is its questionnaire, which has been specifically designed to capture the effects of COVID-19. Data were collected through an online survey distributed to workers in Syria. The survey, which utilized a Likert scale, was sent via email to around 2650 employees and was also shared on social media platforms. A total of 300 respondents took part in the survey. To address the research questions, structural equation modelling was employed in this study. The results indicated that remote work resulted in increased enjoyment, satisfaction, and motivation among employees, ultimately improving job performance. It is worth noting that while this research focuses on examining these benefits within the context of Syria, there is potential for external validity among other countries affected by the COVID-19 pandemic.

Abstract: ABSTRACT Background Previous meta‐analyses have shown mixed results regarding the association between eating disorders (EDs) and type 1 diabetes mellitus (T1DM). Our paper aimed to analyse different EDs and disordered eating behaviours that may be practiced by patients with T1DM. Methods A literature search of PubMed, Scopus and Web of Science was conducted on 17 January 2023, using the key terms “T1DM,” “Eating Disorders” and “Bulimia.” Only observational controlled studies were included. Abstract: Background: COVID-19 is primarily known as a respiratory illness; however, many patients present to hospital without respiratory symptoms. The association between non-respiratory presentations of COVID-19 and outcomes remains unclear. We investigated risk factors and clinical outcomes in patients with no respiratory symptoms (NRS) and respiratory symptoms (RS) at hospital admission. Methods: This study describes clinical features, physiological parameters, and outcomes of hospitalised COVID-19 patients, stratified by the presence or absence of respiratory symptoms at hospital admission. RS patients had one or more of: cough, shortness of breath, sore throat, runny nose or wheezing; while NRS patients did not. Results: Of 178,640 patients in the study, 86.4 % presented with RS, while 13.6 % had NRS. NRS patients were older (median age: NRS: 74 vs RS: 65) and less likely to be admitted to the ICU (NRS: 36.7 % vs RS: 37.5 %). NRS patients had a higher crude in-hospital case-fatality ratio (NRS 41.1 % vs. RS 32.0 %), but a lower risk of death after adjusting for confounders (HR 0.88 [0.83–0.93]). Conclusion: Approximately one in seven COVID-19 patients presented at hospital admission without respiratory symptoms. These patients were older, had lower ICU admission rates, and had a lower risk of in-hospital mortality after adjusting for confounders.

Abstract: Background: real-time polymerase chain reaction (RT-PCR) of nasal/oropharyngeal samples is the gold standard in detecting SARS-CoV-2 infection, however, it has a long turnaround time. Objectives: To meet the growing epidemic demand, we evaluated the performance of rapid SARS-CoV-2 antigen tests in detecting COVID-19 infection compared to RT-PCR. Patients and Methods: a cross-sectional study involving adults suspected to have mild or moderate COVID-19 infection severity scores over 6 months from 1/10/2021 to 1/4/2022. Results: from 186 patients assessed, 88(47.3%) males and 98 (52.7%) females with a mean age of 52.55 ± 20.15 years, and a mean disease duration of 13.1± 4.5 days. 57(30.6%) cases of RT-PCR were positive and 129(69.4%) cases were negative. Meanwhile, in rapid antigen tests, 49(26.34%) cases were positive and 137(73.35%) cases were negative. The receiver operator characteristic (ROC) curve predicts the performance of rapid antigen tests revealing an overall agreement with the RT-PCR results, with 85% sensitivity (95% CI, 73.43% - 92.90 %), 98.54% specificity (95% CI, 94.83% - 99.82%), 96.23% PPV (95% CI, 86.52% - 99.02%), 93.75% NPV (95% CI= 89.14% - 96.48%), 94.42% accuracy (95% CI, 90.23% - 97.18%), and with 0.918 the area under the curve. Conclusion: Although RT-PCR is still the gold standard for detecting SARS-CoV-2 infections, the rapid SARS-CoV-2 antigen test offers good sensitivity, specificity, PPV, NPV, and a significantly short turnaround time. As a result, it has great clinical utility as a primary frontline test for detecting infected patients in an emergency setting.

The purpose of this article is to investigate prospective green chain suppliers for a laurel soap producing company based on a framework using MCDA fuzzy technologies such as Fuzzy GRA, Fuzzy TOPSIS, and Fuzzy VIKOR. During this paper, a discussion of fuzzy set was discussed with Fuzzy MCDM. Within a realistic case study, where a comparison was made between the tools, and it was noted that the results were consistent between the two tools Fuzzy TOPSIS and Fuzzy GRA, and the arrival of similar results and a slight difference with results was observed in Fuzzy VIKOR interpreted. The most detailed analyses were conducted on distance measurements, linguistic variables, assembly functions, and confusion processing. This study introduces the Fuzzy MCDM method to help researchers choose a more effective approach for green supplier selection and the conclusions and other study directions are offered toward the conclusion of the paper.

Abstract: Abstract One of the most important challenges for decision-makers and investors is location selection, which may be assessed using multicriteria decision-making (MCDM) methodologies. Problems with picking a location include deciding between alternative locations, analyzing alternatives, and identifying the best location for a hospital. Because they analyze options with multiple perspectives in terms of numerous competing criteria, MCDM approaches are useful instruments for solving decision-making challenges. The fuzzy set theory (FST), which represents uncertainty in human beliefs, may be effectively used with MCDM approaches to produce more sensitive, tangible, and accurate findings in this context. A hybrid fuzzy multi-criteria decision model (FMCDM) is proposed to find the optimal location based on a combination of factors. In the first stage, the Fuzzy Analytical Hierarchy Process (FAHP) is used to estimate the relative criteria classification through the evaluation process. In the second stage, the fuzzy technique of order preference using similarities to the perfect solution (FTOPSIS) is applied to rank the possible alternative sites. The findings from this study indicate that integrate FAHP and FTOPSIS is the most often used FMCDM approach in Aleppo for selecting the best location for a new hospital.

Abstract: Abstract Background Pediatric onset multiple sclerosis (POMS), defined as an age at onset younger than 18 years, which occurs in 5% of patients with MS. cognitive dysfunction is one of the prominent disabling sequelae of Multiple sclerosis. Brain volumetric studies by magnetic resonance images revealed the decline of whole and regional brain volumes along the disease course. This work aimed to investigate the relationship between cognitive impairment in pediatric MS patients with thalamic atrophy and cortical thickness of temporal lobe. This study included 50 patients who were diagnosed as POMS and 50 healthy control participants matched for age and sex. Both groups were compared for volumetric measurements of thalamic volumes and temporal lobes cortical thickness using a computerized program called FreeSurfer.MS group was evaluated for cognitive dysfunction using Arabic version of fifth edition of Standford–Benit test. A correlation between volumetric results and neuropsychological evaluation of MS group was done. Results Our study showed that the MS group has the lowest value regarding their thalamic volumes and their cortical thickness of temporal lobes in relation to the healthy control group, while there was a significant relation between cognitive impairment and decrease in thalamic volume and specific areas in cortical thickness, such as superior temporal thickness, middle temporal thickness, inferior temporal thickness, fusiform thickness and para hippocampal thickness of temporal lobe in pediatric onset MS patients. Conclusions POMS affects specific brain areas such as thalamus and cortical thickness of temporal lobes regarding their volume and thickness which influence the neuropsychological evaluation detected by Standford–Benit test.

Abstract: Abstract Our study aimed to evaluate the levels of MDSCs and Tregs in pediatric B-cell acute lymphoblastic leukemia (B-ALL), their relation to patients’ clinical and laboratory features, and the impact of these cells on the induction response. This study included 31 pediatric B-ALL patients and 27 healthy controls. All patients were treated according to the protocols of the modified St. Jude Children’s Research Hospital total therapy study XIIIB for ALL. Levels of MDSCs and Tregs were analyzed using flow cytometry. We observed a reduction in the levels of CD4 + T-cells and an increase in both the polymorphonuclear MDSCs (PMN-MDSCs) and Tregs. The frequencies of PMN-MDSCs and Tregs were directly related to the levels of peripheral and bone marrow blast cells and CD34 + cells. Complete postinduction remission was associated with reduced percentages of PMN-MDSCs and Tregs, with the level of PMN-MDCs in this subpopulation approaching that of healthy controls. PMN-MDSCs and Tregs jointly play a critical role in maintaining an immune-suppressive state suitable for B-ALL tumor progression. Thereby, they could be independent predictors of B-ALL progress, and finely targeting both PMN-MDSCs and Tregs may be a promising approach for the treatment of B-ALL.

Abstract: This study analyzes the implementation of Egypt's targeted fiscal stimulus to the irregular workforce in response to the COVID-19-induced partial shutdown in 2020. We utilize theoretical insights from the literature on administrative campaigns and the interagency coordination literature, as well as insights from the polycentric governance literature, to identify the approach to coordinating the work of the multiple agencies that participated in implementing the initiative and situate it within the implementation literature. We conclude that the complexity of public administration initiatives in semi-authoritarian regimes could create a pretext for expanding access to decision-making and implementation. The need for input from diverse actors creates an environment conducive to expanding participation that combines the characteristics of top-down and bottom-up approaches to policy implementation. Access to decision-making and implementation structures by non-state actors, however, remains dependent on relations with state bureaucratic agencies, both civilian, security-, and military-affiliated. The regime's interest in maintaining control, especially control over data, and the participants' attention to following the official line, creates a need for a dominant actor to perform certain functions beyond the ability of the participating members to accomplish. Abstract: Dairy products can be contaminated from their surrounding surfaces and equipment with biofilm forming foodborne pathogens such as Escherichia coli (E. coli). Biofilm production is regarded as an adaptive mechanism that enhance microbial growth and proliferation. In sight of these facts, this study aimed to investigate of the prevalence of E. coli in dairy products (kariesh cheese, ice cream, and pasteurized milk), and dairy plant floors, and their contact equipment. Besides, the abilities of the identified E. coli serotypes to produce biofilm were further examined. In a reduction trial, the use of Nigella sativa and olive oils’ nanoemulsions to reduce E. coli-biofilm formation was examined. The obtained results indicated that the prevalence rates of E. coli in the examined kariesh cheese, ice cream, pasteurized milk, dairy plant floors, and equipment were 19.05%, 4.76%, 0%, 45.24%, and 30.96%, respectively. Five E. coli serotypes were identified in the current investigation namely, E. coli O26:H11, E. coli O55:H7, E. coli O78:H-, E. coli O111:H4, and E. coli O127:H6 at variable rates. Molecular identification of the recovered E. coli serotypes revealed that all serotypes harbored E. coli-specific 16S rRNA, and fim H, biofilm-coding genes. All identified E. coli serotypes had the ability to produce biofilm in the following order: E. coli O26:H11, E. coli O55:H7, E. coli O78:H-, E. coli O111:H4, and E. coli O127:H6, respectively. Nigella sativa, and olive oils’ nanoemulsions reduced biofilm production in a concentration dependent manner.

The purpose of this study was to investigate the impact of mobile Virtual interactive e-learning environment on 3D operating system course learning motivation. The paper aims to increase students’ interest and enhance their learning motivation for understanding CPU process scheduling algorithms in an operating systems course. A total of 110 students from the department of computer sciences at Sadat Academy were invited to participate. A 5-point scale was adopted based on the ARCS model to stimulate learning to collect student data. A one-way analysis of variance (ANOVA) was performed to assess if the apparent difference was significant. Perform a data factor analysis to determine the appropriate factor (p = 0.000 < 0.05). The results show that mobile virtual reality game-based learning is an effective pedagogical tool for operating systems education, and it can promote students’ motivation and interest in learning 3D animation.

Abstract: Objective: directed to evaluate to Effect of Casein Phospho PeptideAmorphous Calcium Phosphate on Some Salivary Ions Concentration in Children with Early Childhood Caries in comparison with the use CPP-ACPF and sodium fluoride 5% varnish. Patients and methods: A total of 36 children with early child-caries (ECC), aged between 2.5-5 years were selected for this clinical trial study. Registered children were split into three groups at random according to the tested materials, group A; treated with non-fluoridated CPP-ACP-paste, group B; treated with CPP-ACPF-paste, and group C; treated with 5% sodium fluoride (NaF) paste. At first, 1 ml of unstimulated saliva was collected from whole enrolled children. In the next step, each tested material was applied on the tooth surfaces and then the saliva sampling was performed after 48 hours. The amount of phosphate ions was measured by spectrophotometer, while, calcium, and fluoride ions were measured by chromatographic device. Data were analyzed utilized paired t-test also used One-way ANOVA test at a p Results: This study revealed that; significant increase in phosphate and calcium ions condensation of the saliva before and after applying the CPP-ACP and CPP-ACPF pastes. However, the applying of CPP-ACPF paste, and 5% sodium fluoride paste significantly raise the fluoride scores of the saliva. Conclusion: In this study, the utilize of CPP-ACP also the use of CPP-ACPF pastes can easily the remineralization of tooth surfaces and is beneficial for protecting the primary teeth.

Abstract: The objective of this study was to evaluate the effects of Pediococcus acidilactici (P. acidilactici) and Saccharomyces cerevisiae (S. cerevisiae) on the growth performance, intestinal lesions and ileal Clostridium perfringens (C. perfringens) count of broiler chickens challenged with C. perfringens induced sub-clinical necrotic enteritis (NE). A total of 150 broiler chicks, allocated into five treatment groups (6 replicates of 5 chicks/cage), were reared in cages for 29 days: T0 (control group, not infected, not supplemented); T1 (infected with C. perfringens, not supplemented); T2 (infected with C. perfringens and coccidia, not supplemented); T3 (infected with C. perfringens and coccidia, supplemented with P. acidilactici); T4 (infected with C. perfringens and coccidia, supplemented with S. cerevisiae). The parameters analyzed were: body weight gain, feed intake, feed conversion ratio, mortality, intestinal lesion score and ileal C. perfringens enumeration by quantitative real-time PCR. The challenge resulted in impairment of growth performance, increased lesion score (≤ 2) and overgrowth of the C. perfringens population. However, the dietary inclusion of P. acidilactici or S. cerevisiae caused a significant improvement in feed conversion, net reduction of gut lesions, as well as a decrease in the intestinal C. perfringens population. In conclusion, these results suggest that dietary supplementation with probiotics (P. acidilactici or S. cerevisiae) could be beneficial to alleviate the negative effects of subclinical NE in broiler chickens.

Abstract: Background: ST-elevation myocardial infarction (STEMI) is considered one of the commonest causes of morbidity and mortality all over the world. Nevertheless, survival following acute STEMI showed a considerable improvement as a result of increased symptom recognition, precise diagnosis in addition to effective timely reperfusion. Objective: To detect the significance of the reciprocal ST segment alterations in the early stage of acute myocardial infarction (AMI) and whether it is truly reciprocal or represents a distant ischemia that might be revealed by coronary angiography. Patients and methods: This study was conducted on 133 patients admitted to Al-Mokattam Health Insurance Hospital, with the diagnosis of acute STEMI treated with primary percutaneous coronary intervention (PPCI) during the period between November 2018 and December 2019. Results: Distant ischemia in patients with STEMI undergoing PPCI level was significantly higher in reciprocal ST segment depression (RSTD) group (p-value < 0.001). Besides, multivessel disease (MVD) was more prevalent in RSTD group (36.8% vs. 17.5%) in comparison with NRSTD group. The degree of mitral regurge (MR) was more severe among patients with RSTD (P value = 0.029). Mortality rate was more in patients with RSTD, six patients has passed away all of them had RSTD. Conclusion: The reciprocal changes (RC) caused by interaction of distant ischemia of non-infarcted coronaries and benign mirror electrical alterations, and it has a mortality effect. So, RSTD that was a simple ECG finding used as an indicator of patients at high risk who require a more aggressive approach.

Abstract: Background and aim The data about the role of regulatory B cells (Breg) in Behcet Disease (BD) are scarce. We aimed to evaluate the frequency of total B lymphocytes and Breg cells in different BD phenotypes and therapies attempting to unravel their function. Methods This cross-sectional study included 35 BD patients and 39 healthy controls (HCs). The demographic data of the study subjects were collected including age and gender. Current medications including disease-modifying anti-rheumatic drugs (DMARDs) were recorded. All patients underwent testing for baseline laboratory investigations including full blood count, liver and kidney function tests, erythrocyte sedimentation rate (ESR) by Westergren blot and C-reactive protein (CRP). Measurement of the total B lymphocytes and their subtypes B regulatory lymphocytes by flow cytometric assay. Assessment of BD activity was done using the revised Behcet's Disease Current Activity Form (BDCAF) 2006 and Behcet's Syndrome Activity Score (BSAS) 1111111111. All participants were assessed for the presence of erectile dysfunction using the International Index of Erectile Function (IIEF-5 score), and for depression using the Beck Depression Inventory. Results A dramatic drop in the number of B cells, total and regulatory, was observed in the patients compared to the HCs. Regulatory cells (Bregs) tend to be upregulated with genital ulcers or vascular disease. Bregs but not B lymphocytes were associated with BSAS and ESR. Neither the total B lymphocytes nor the Bregs correlated with CRP or the sexual function or depression scores. Of all the used medications, low-dose aspirin was seen with markedly high Bregs proportions. Conclusion This study supports the role of B cells in BD pathogenesis and strongly suggests a possible role for Bregs in the resolution of different BD manifestations.

Abstract: This study was undertaken to investigate the efficiency of partial replacement of soybean meal (SBM) by Jatropha curcas meal treated with lactobacillus acidophilus at rate of 1g/100kg. Four experimental diets were formulated, as iso nitrogenous (30.5 % crude protein) and isocaloric (4543.56Kcal/kg diet), containing 0, 25, 50 and 75% of treated Jatropha meal (TJM) T1, T2, T3 and T4, respectively and fed to monosex Nile tilapia, (Oreochromis niloticus) fingerlings with average weight (30.43±1.14g/fish).Total of 60 fingerlings were random distributed in four treatments, triplicate groups each with five fish/ aquarium. The experiment period was lasted for 70 days. Growth performance, feed utilization and body compositions as well as the economic evaluation were monitored. The obtained data showed that, higher weight gain and the best feed conversion ratio (FCR), protein efficiency ratios (PER), protein productive value (PPV % ) and energy utilization (EU%) recorded for ﬁsh fed diets (up to 50 %) of treated Jatropha meal were insigniﬁcantly different from those fed soybean meal. It could be concluded that the SBM can be partial replaced with TJM up to50 % of SBM without any adverse effects on growth performance or feed utilization of Nile tilapia.

Abstract: Background Acne vulgaris is a multifactorial inflammatory skin disease of the pilosebaceous unit with complex pathogenesis. Calprotectin is a heterodimer of two calcium-binding proteins S100A8 and S100A9, which are members of the S100 protein family, and which play a role in various inflammatory processes, it is mainly present in the cytoplasm of neutrophils and expressed on the membrane of monocytes. Objective to assess the level of Calprotectin in blood and in tissue of AV patients and compare it with healthy controls. Beside correlate it with the degrees of severity of acne in order to further understand the role of Calprotectin in the pathogenesis of AV. Subjects and Methods This case-control study included 28 subjects who have AV, divided into four groups according to Global Acne Grading System: the first group included 7 patients with mild acne, the second group included 7 patients with moderate acne, the third group included 7 patients with severe acne, and the fourth group included 7 patients with very severe AV. In addition to a control group which included 28 apparently healthy individuals of matched age, sex and with no previous history of acne or active acne. All the patients were recruited from the outpatient clinic of Al Haud Al Marsoud hospital from September 2019 till January 2020. After taking the approval of research ethics committee (FMASU M S 22/2019) all subjects gave an informed consent to participate in this work. Results Our study showed that the increase in AV severity was not affected by the difference in gender, type of acne, nor disease duration. However, there was a significant correlation between plasma calprotectin level and AV severity. In addition to, There was a significant statistical correlation between the increase in the age of patients and the decrease in the severity of acne, which indicates that AV is more severe in younger patients than older ones. The correlation between the Plasma Calprotectin level in patients group and Tissue Calprotectin level in inflammatory lesions (papules) was non-significant. However, the correlation between the Plasma Calprotectin level in patients’ group and Tissue Calprotectin level in non-inflammatory lesions (comedones) was highly significant. Conclusion Plasma Calprotectin level showed significant relation with AV severity and presence of scar. The correlation between plasma Calprotectin level and tissue Calprotectin level in noninflammatory lesions (comedones) was highly significant.

Abstract: Introduction There has been an increasing recognition that cardiac surgery patients have different resuscitative needs than other medical and surgical patients who experience in-hospital cardiac arrest. This was addressed in the 2010 European Resuscitation Council Guidelines and the 2010 American Heart Association Guidelines. However, it’s unknown how widely the guidelines are practiced, or a training protocol is followed in different units in Egypt. Objectives This national survey aims to identify the views and common practice of Egyptian cardiac teams regarding resuscitation after cardiac surgery. Methods A 21-question survey is created based on a prior survey used by the EACTS guidelines committee. Questions included the following topics: Participants demographics, Prevalence of cardiac arrest in the intensive care unit, Cardiac arrest with ventricular fibrillation or non-shockable rhythm, Emergency resternotomy technique, Training and arrest protocols. Survey dissemination was through social media platforms, mobile messaging applications and emails during the time period between November 2020 and January 2021. Results Of 126 responses, 95 were suitable for inclusion. Responses were from 11 centres across Egypt. 68.5% of the respondents were surgeons while cardiac anaesthetists and intensivists formed 12.6% and 18.9% respectively. 76.8% of participants were middle-grade doctors, consultant participation was 23.2%. The median annual number of cases performed in the units was 480 and this ranged from 10 to 3000. The average percentage of cardiac arrests was 7%, Median survival to hospital discharge of all arrests was 33% For patients who go into VF after cardiac surgery, respondents would attempt a mean of 5 shocks with only 24.2% commencing defibrillation shocks before external cardiac massage, while the majority initiating CPR immediately. They would perform emergency resternotomy in a mean time of 12 mins and in 15 mins if the rhythm was not shockable. 56.8% would give 1 mg of adrenaline as soon the cardiac arrest was established, only 6.3% thought that it should be given rarely or not at all. If a surgeon was not immediately available 36.8% of respondents would be happy for any trained personnel to perform the emergency resternotomy while 58.9% expect only the surgeon to perform the resternotomy. 49.4% of the participants have not practised any training to perform an emergency sternotomy. 41% of the respondents state they occasionally practice or talk with the staff about it. Only 9.5% practice regularly on emergency sternotomies. 25% assume current training is enough and does not need modification, while 75% think tailored training is important and staff should be oriented about it in the future. Conclusion An action plan is required to improve the awareness of the junior surgeons with the Cardiac Advanced Life Support Protocol. Proper training of the intensive care staff to implement the protocol in a timely organised manner is needed. Assessment of the rhythm before starting external chest compressions is the corner stone in cardiac patients undergoing cardiac arrest. VF/pVT rhythm, 3 defibrillation shocks are given first; for the non- shockable rhythm, the emergency pacing switched on, this is followed by compressions till resternomtomy. Emergency resternotomy under 5 minutes is the only effective way to save patients with tamponade and extreme hypovolemia.

The enrolled patients fulfilled the inclusion and exclusion criteria and were divided to two groups o Study group: All patients who were diagnosed with U.C. based on clinical history, laboratory and endoscopic finding according to ECCO 2017 guidelines (Sandro et al., 2017). o Control group: Historical Control samples from healthy Egyptian subjects. Both groups were compared according to basal epidemiologic data and H. pylori Ag in stool positivity, gastrointestinal symptoms, colonscopic and microscopic findings, mainstay treatment lines of U.C., biological therapy and criteria of disease activity . Results Most of the enrolled patients in the current study were aged between 20-39 years with 48.6% aging between 20-29 years old and 38.6% between 30-39 years. While the majority of the cases were females (62.9%), only 11.4% of the included cases were smokers . We found that of the 70 patients in the cases group with ulcerative colitis, only 34 (48.6%) had positive H. pylori versus 555 reported positive samples in control group (91.7%). So there was a high significant statistical difference between the 2 groups Conclusion There is a significant association between H. pylori infection and ulcerative colitis that supports a possible protective benefit of H. pylori infection against the development of U.C. but still further studies are needed
