Abstract

BackgroundSuccessful chronic total occlusion (CTO) percutaneous coronary intervention (PCI) is known to be associated with improved clinical outcomes compared with failed CTO PCI. However, it is not clear whether the angiographic and clinical outcomes of long CTO lesionis different with those of short CTO lesion in the drug eluting stent (DES) era.Method sand ResultsA total of 235 consecutive patients underwent successful CTO intervention were divided into two groups according the CTO lesion length. Six-month angiographic and two-year clinical outcomes were compared between the two groups. The baseline clinical characteristics were similar between the two groups except prior PCI was more frequent in long CTO group whereas bifurcation lesion was more frequent in the short CTO group. In-hospital complications were similar between the two groups except intimal dissection was more frequent in long CTO group. Both groups had similar angiographic outcomes at 6 months and clinical outcomes up to 2 years except the incidence of repeat PCI, predominantly target vessel revascularization (TVR) was higher in long CTO group. In multivariate analysis, long CTO was an important predictor for repeat PCI (OR;4.26, CI 1.53-11.9, p=0.006).ConclusionThe safety profile, angiographic and 2-year clinical outcomes were similar between the two groups except higher incidence of repeat PCI in long CTO group despite of successful PCI with DESs.

Abstract

Background: Post-stenting balloon dilation is associated with improvement of stent deploymanet and better angiographic results in percutaneous coronary intervention (PCI). However, it had not been evaluated competently in setting of acute coronary syndrom (ACS) undergoing PCI. There are limited data regarding the impact of post-stenting balloon dilatation on long term clinical outcomes in ACS patients. Methods: Procedural details of total 2533 ACS patients (693 ST elevation myocardial infarction, STEMI patients, 640 Non-ST elevation MI, NSTEMI patients, and 1220 Unstable Angina patients) underwent PCI were examined. Patients were divided into two groups; 1) NO Post dilatation group (N=1047) and 2) Post dilatation group (N=1506). Individual and composite major clinical otucomes including major adverse cardiac events (MACE), the composite of total death, MI,stroke and revascularization, were compared between the two groups up to 5 years. Results: At 5 years follow up, post dilatation group had signficantly lower target vessel revascularization (TVR, HR: 0.716, 95% CI: 0.524 to 0.978 ;p = 0.035), non-TVR (HR: 0.538, 95% CI: 0.319 to 0.908; p = 0.020), stroke (HR: 0.778, 95% CI: 0.638 to 0.948; p = 0.013) and MACE (HR: 0.749, 95% CI : 0.592 to 0.947; p = 0.016) than No post dilatation group (Table). Using multivariate logestic regression, and after adjusting for potential risk factors, post diltation patients had lower total MACE (HR: 0.711, 95% CI : 0.524 to 0.965; p = 0.029), including TVR and stroke. Abstract

Background: Percutaneous coronary intervention (PCI) for ostial coronary lesions associated with inappropriate angiographic results, challenging adjustment of stent position and higher risk of cardiovascular complications. However there are limited data regarding the impact of ostial stenting on short and long-term clinical outcomes. Methods: A total 3747 patients underwent PCI were included. Patients were divided into two groups; 1) non-ostial lesion group (N= 2936) and 2) ostial lesion group (N=811). Due to potential confounders, a propensity score matching (PMS) analysis was performed using the logistic regression model. Individual major clinical outcomes and major adverse cardiac events (MACE), the composite of total death, myocardial infarction (MI), stroke, stent thrombosis and revascularization, were compared between the two groups at 30-day and up to 5 years. Results: After PSM analysis, two matched groups (712 pairs, n=1424) were generated and the baseline characteristics were balanced. At 30 days, ostial lesion group had higher mortality including cardiac death (HR: 0.276, 95% CI: 0.103 to 0.744; p = 0.011), revascularization (HR: 0.355, 95% CI: 0.128 to 0.985; p = 0.047) and all MACE (HR: 0.510, 95% CI: 0.284 to 0.915; p = 0.024) than the Non- ostial lesion group. At 5 years, the ostial lesion group patients also had higher mortality including cardiac death (HR: 0.423, 95% CI: 0.194 to 0.923 ; p = 0.031), revascularization (HR: 0.686, 95% CI : 0.447 to 0.986 ; p = 0.042), target vessel revascularization TVR (HR: 0.604, 95% CI : 0.405 to 0.902 ; p = 0.014) and all MACE (HR: 0.683, 95% CI : 0.507 to 0.919 ; p = 0.012) than the Non- ostial lesion group (Table). Conclusions: This study implies that the presence of coronary ostial lesion is an independent risk factor for adverse clinical outrcomes including higher incidence of cardiac death, revascularization and total MACE at 30 days and up to 5 years. Abstract

Background:Lesion length had been investigated widely as a strong predictor of low procedural complications, high complication rate and high restenosis rate after percutaneous coronary intervention (PCI). However, there are limited data regarding the long-term clinical outcomes following successful PCI for long lesions in the setting of acute myocardial infarction (AMI).Methods:A total 1302 AMI patients underwent PCI were enrolled. Patients were divided into two groups; 1) long lesion group (?30mm, N=737) and 2) short lesion group (<30mm, N=565).Individual and composite major clinical outcomes including major adverse cardiac events (MACE), the composite of total death, myocardial infarction (MI), repeat revascularization and stent thrombosis were compared between the two groups up to 5 years.Results:At 5 years follow up, long lesion group had significantly higher incidence revascularization (HR: 1.377, 95% CI: 1.030 to 1.840; p = 0.031) including target lesion revascularization (TLR, HR: 1.620, 95% CI: 1.137 to 2.308; p = 0.008) and MACE (HR: 1.338, 95% CI: 1.062 to 1.685; p = 0.014) than short lesion group (Table). Using multivariate logistic regression, and after adjusting for potential risk factors, long coronary lesion patients had higher incidence of MACE including total death, cardiac death, revascularization and stent thrombosis up to 5 years.Conclusion:In this study, long coronary lesions (?30mm) treated with drug-eluting stents after AMI were associated with higher incidence of revascularization and total MACE than short coronary lesion (< 30 mm) up to 5 years.

Abstract

Background:Dual antiplatelet therapy including aspirin and clopidogrel had been indicated for inhibition of platelets activity after percutaneous coronary intervention (PCI) in the setting of acute myocardial infarction (AMI). However, there are limited data concerning the impact of triple antiplatelets (including cilostazol) on short and long term clinical outcomes following multivessel PCI in AMI setting.Methods:A total 314 patients underwent PCI in 2 or more vessels were included. Patients were divided into two groups; 1) dual antiplatelet therapy (N=197) and 2) triple antiplatelet therapy (N=117).Individual and composite major clinical outcomes including major adverse cardiac events (MACE), the composite of total death, myocardial infarction (MI), repeat revascularization and stent thrombosis were compared between the two groups up to 1 year.Results:The baseline clinical characteristics were similar between the two groups except male gender , dyslipidemia and peak CK-MB were higher in triple therapy group. However, male gender was higher in dual therapy group .Triple therapy group had higher incidences of ostial lesions, bifurcation lesions, and left main stenosis. Peak values of troponin T, bleeding complications, and LVEF were similar between the two groups. At 30-day follow up, the incidence of total MACE (HR: 0.960, 95% CI: 0.424 to 2.171; p = 0.921) between triple and dual antiplatelet group were similar. Moreover, at 1-year follow up, the incidence of MACE (HR1.240, 95% CI: 0.776 to 1.981; p = 0.368) not different between the two groups.Conclusion:In this study, triple antiplatelet therapy following multivessel PCI in AMI was associated with reduced risk of adverse clinical outcomes including incidence of cardiac death, revascularization, stent thrombosis, stroke, stent thrmbosis and total MACE as compared with dual antiplatelet therapy despite of worse clinical and angiographic characteristics at 30 days and up to 1 year.
