MANTEGAZZA, RENATO, open, Amyotrophic Lateral Sclerosis (ALS) is a neurodegenerative and fatal disease characterized by progressive cortical, bulbar and spinal motor neuron (MN) degeneration, leading to progressive muscle weakness, atrophy, paralysis and, ultimately, death. ALS can occur in two different forms: sporadic ALS (sALS) in ∼90% of individuals and familial ALS (fALS). Different genes have been associated with fALS and/or sALS; C9ORF72–SMCR8 complex subunit (C9ORF72) is the gene most commonly linked to inherited ALS, followed by TAR DNA-binding protein 43 (TARDBP), superoxide dismutase 1 (SOD1) and FUS RNA-binding protein (FUS). Such genes affect several cellular functions, including oxidative stress (SOD1), RNA metabolism (C9ORF72, TARBDP and FUS), cytoskeletal organization [e.g. tubulin alpha-4a (TUBA4A) and profilin 1 (PFN1)] and autophagy [e.g. TANK-binding kinase 1 (TBK1) and optineurin (OPTN). ALS-associated mutant genes are ubiquitously expressed, thus alterations in structure, metabolism and physiology occur in different cell types, synergistically contributing to ALS degenerative pathways. It is generally accepted that ALS is primarily caused by MN death. However, growing evidence has shown that muscle is active and plays a crucial role in the disease onset and progression. Currently, there are no effective treatments for ALS. Indeed, one of the major aims in ALS research is the development of successful therapies, by deepening the knowledge of the molecular events leading to the degeneration of both MNs and muscle tissue. It has become increasingly clear that RNA dysregulation is a key contributor to ALS pathogenesis. Among non-coding RNAs, long non-coding RNA (lncRNAs) are emerging as molecular contributors to ALS pathophysiology because of their role in regulating gene expression. LncRNAs, that are 300 to thousands nucleotides long, being more similar to mRNA than microRNAs, are key MN and muscle gene expression regulators. Ad oggi non esistono cure e uno degli obiettivi della ricerca è lo sviluppo di terapie, ottenute tramite una conoscenza specifica degli eventi molecolari che portano alla degenerazione dei MN e del tessuto muscolare. La deregolazione dell’RNA ha un contributo chiave nella patogenesi della SLA. Nel campo dell’RNA non coding, i long non coding RNA (lncRNA) emergono come contribuenti alla patofisiologia della SLA. LANZUOLO, CHIARA, open, Prostate cancer (PCa) is the second most recurrent male tumor, often characterized by an unfavorable outcome due to its high clinical and molecular heterogeneity and its frequent multifocality. Increasingly growing epigenetic research efforts are implicated in the detection of novel biomarkers improving PCa diagnosis and prognosis. Among the epigenetic layers, the chromatin nuclear architecture abides tight rules that ensure proper genome functions and cell identity maintenance, and its remodeling has emerged as a novel crucial player during malignant transformation. Yet, the occurrence and the functional implications of chromatin structural changes in cancer remains poorly characterized. We have recently developed a high-throughput sequencing based technique, named 4fSAMMY-seq (4 fractions Sequential Analysis of MacroMolecules accessibilitY), consisting of the sequential fractionation of the chromatin according to its accessibility and solubility status to isolate and characterize both heterochromatin and euchromatin. A key advantage of our method is that it only requires few thousands of living cells, making it suitable for analyses on limited materials as biopsy samples derived from clinical practice. In this study, we applied 4fSAMMY-seq on fresh biopsy specimens from patients undergoing explorative prostate mapping biopsy for prostate cancer diagnosis. We examined the genome conformation and its association with transcriptional profile in control tumor-free biopsies and prostate cancer biopsies. In bulk healthy tissues, containing a mix of epithelial cells, leukocytes and stroma, 4fSAMMY-seq allowed the detection of a conserved pattern of euchromatin and heterochromatin regions associated with the respective epigenetic signatures. BODEGA, BEATRICE, open, Tumor infiltrating lymphocytes (TILs) are the principal components of the tumor microenvironment, and play a central role in antitumor immunity. During cancer immunoediting, TILs became dysfunctional, a phenotype associated to effector functions impairment, reduced cell growth and decreased killing capability. Indeed, cancer immunotherapies treatments try to revert TILs dysfunctional state in order to promote tumor clearance. Immunotherapies nowadays represent the novel frontier in fighting cancer but still little is known regarding the epigenetic modulators responsible for TILs properties; the discovery of novel, possible targetable molecules and mechanisms could substantially improve knowledges regarding immunotherapies and patient responsiveness to them. For this reason, we are applying novel approaches and technologies in this filed, namely the investigation of transposable elements (TEs) functions as novel epigenetic players in TILs identity, plasticity and adaptability to the environmental cancer driven milieu. TEs are interspersed repetitive DNA sequences that cover 40 - 45% of the human genome and growing evidence suggests that TEs exert a crucial function in epigenetic regulation both in cis and in trans, being a source of non-coding regulatory RNAs and participating to chromatin folding. Among TEs, we are interested in the possible epigenetic functions of LINE1 elements, that represents 18% of the human genome, considered as novel key molecules involved in epigenetic regulation of cell identity. Until today the role and the dynamics of TEs-derived RNAs were investigated only in embryonic stem cells or during organism development, while there was no evidence of their possible functions in fully differentiated cells derived from adult tissues, as human Naïve T lymphocytes, that are plastic cells able to adapt and differentiate to diverse effector cells based on the cytokine milieu. ASSELTA, ROSANNA, open, The main goal of this PhD project was to leverage the great body of genetic knowledge generated by high-throughput DNA sequencing to better characterize the genetic architecture of immune diseases, with a main focus on Primary Biliary Cholangitis (PBC). Our work has employed established statistical methods such as meta-analysis as well as novel data mining tools, such as packages dedicated to the study of chromosome X (chrX) and Machine Learning (ML) softwares. In addition, we have also applied established computational methods and ML tools to shed light on the evolutionary history of variants associated with immune-mediated traits. More specifically, the meta-analysis of previous Genome-Wide Association Studies (GWAS) in PBC has identified additional risk loci and, by means of functional annotation of credible causal variants and multi-omic analysis, has produced a list of candidate genes together with several drugs that are potentially suitable for re-purposing to PBC. The extensive analysis of chrX has identified several suggestive new loci associated with PBC located on this chromosome. The major finding of the X-Wide Association Study (XWAS) approach has been the identification of a genome-wide significantly associated locus characterized by the presence of different genes and of a superenhancer possibly involved in their co-regulation, as well as in the regulation of FOXP3 (which is located in the same TAD). Based on the new set of variants identified in the international meta-analysis, we have generated a novel Polygenic Risk Score (PRS) that has been incorporated in a new integrative risk model. This model, that has included 22 non-HLA variants, one HLA variant and sex, is accurate (Area Under the Curve (AUC) 0.83 and 0.81 in the two cohorts under study) and well calibrated. The PRS has pinpointed a subgroup of subjects at strikingly higher risk (OR ~ 14) of developing the disease that should be the target of tailored follow-up strategies. 9, Citerio Giuseppe the CENTER-TBI Collaborators, open, Background: Traumatic brain injury (TBI) is an extremely heterogeneous and complex pathology that requires the integration of different physiological measurements for the optimal understanding and clinical management of patients. Information derived from intracranial pressure (ICP) monitoring can be coupled with information obtained from heart rate (HR) monitoring to assess the interplay between brain and heart. The goal of our study is to investigate events of simultaneous increases in HR and ICP and their relationship with patient mortality..Methods: In our previous work, we introduced a novel measure of brain-heart interaction termed brain-heart crosstalks (ct(np)), as well as two additional brain-heart crosstalks indicators [mutual information (mi(ct)) and average edge overlap (omega(ct))] obtained through a complex network modeling of the brain-heart system. These measures are based on identification of simultaneous increase of HR and ICP. In this article, we investigated the relationship of these novel indicators with respect to mortality in a multicenter TBI cohort, as part of the Collaborative European Neurotrauma Effectiveness Research in TBI high-resolution work package.Results: A total of 226 patients with TBI were included in this cohort. The data set included monitored parameters (ICP and HR), as well as laboratory, demographics, and clinical information. The number of detected brain-heart crosstalks varied (mean 58, standard deviation 57). The Kruskal-Wallis test comparing brain-heart crosstalks measures of survivors and nonsurvivors showed statistically significant differences between the two distributions (p values: 0.02 for mi(ct), 0.005 for ct(np) and 0.006 for omega(ct)). PAREYSON, DAVIDE, open, Background In adults, many cases (about 30-40%) of leukoencephalopathies (LKENs), i.e. white matter (WM) diseases, are without definitive diagnosis. Patients who remain undiagnosed despite extensive investigations may have atypical forms of known acquired or genetic diseases, or novel diseases more likely genetic in nature. Aims of our work were to explore the efficiency of a systematic approach, including next generation sequencing (NGS), in the diagnosis of a cohort of adult patients with LKEN of unknown cause, and to describe their clinical features. Patients and Methods In this analytical observation study, we first reviewed the clinical and laboratory features of the adult patients (age >= 18 years) with undiagnosed LKEN assessed at the Unit of Rare Neurodegenerative and Neurometabolic Diseases of the Istituto Neurologico “C. Besta”, Milan, Italy, from 2012 to 2018. A targeted-gene panel sequencing (TGPS) was subsequently used to investigate 142 genes responsible for genetic LKENs, and a whole-exome sequencing (WES) was performed in one familial case remained undiagnosed. Results We identified 57 adult patients with LKEN of unknown cause (mean age 43 years, range 18-72; 23 males; 53 with late-adolescence or adult-onset). Thirty of them, henceforward called hypomyelinating leukoencephalopathies (HypoLKENs), presented an MRI pattern suggestive of hypomyelination (mild T2-hyperintensity and normal T1 signal), whereas the remaining 27 (henceforward called demyelinating leukoencephalopathies, DemLKENs) had an MRI pattern suggestive of demyelination (prominent T2-hyperintensity and prominent T1-hypointensity). In 13 HypoLKENs, TGPS identified the disease-causing genes, i.e., POLR3A (n = 2), POLR1C, TUBB4A, RARS1, GJA1, PLP1, GJC2, TBCD, CYP7B1, SPG11, PEX3, and PEX13, while in two further patients, WES led to the identification of a novel disease-causing gene (preliminarily called GENE_A). open, The research activity during the doctorate was focused on an evaluation of HTA for a new drug for the treatment of heart failure. A first pharmaco-economic analysis was conducted before being placed on the market to demonstrate its cost-effectiveness, using data from randomized clinical trials and national rates and is part of the work carried out during the first year of the doctorate. Two new analyzes were subsequently carried out, which aim to demonstrate how pharmaceutical technology, once placed on the market, is still cost-effective and what its impact on the budget of the National Health Service is. In this case, real life data were used as efficacy data and national tariff rates for costs. The budget impact analysis makes it possible to obtain estimates on the economic sustainability of health technologies. The analysis aims to evaluate the financial impact on the Italian National Health Service (NHS) linked to the use of sacubitril / valsartan as therapy for heart failure in NYHA class II and III patients with reduced ejection fraction (FE <35%). The model estimates the target patient pool for the next three years starting from 2021. The target pool is then divided among the therapeutic options currently available. The second health and economic evaluation was conducted with the aim of examining the cost-effectiveness profile of sacubitril / valsartan (in combination with standard therapy), in comparison to enalapril (in combination with standard therapy), in the treatment of chronic heart failure with reduced ejection fraction in Italy A Markov decision model was developed to predict the effects of the two treatment options in comparison, in patients with chronic heart failure and with reduced ejection fraction, in terms of mortality (cardiovascular and all-cause), hospitalizations and quality of life. BROCCOLI, VANIA, COLASANTE, GAIA, open, Dravet syndrome (DS) is a catastrophic developmental and epileptic encephalopathy characterized by severe, pharmaco-resistant seizures and a high risk of Sudden Unexpected Death in Epilepsy (SUDEP). To date, no cure is effective in controlling seizures. 80% of the patients present heterozygous loss-of-function mutations in the SCN1A gene, indicating that a haploinsufficient mechanism underlies the onset of the pathology. SCN1A encodes for the voltage-gated sodium channel alpha-subunit Nav1.1, essential to initiate action potentials (APs) in gabaergic interneurons (GINs). The analysis of different animal models pointed out that seizure development is due to a reduction of excitability of GINs, particularly of parvalbumin (PV), somatostatin (SST), and vasoactive intestinal peptide (VIP) subtypes, ultimately resulting in an over-excitation of neuronal network. However, other works also reported hyperexcitability in excitatory neurons (ExNs) during all stages of the pathology or only in the pre-epileptic stage, suggesting the involvement of this neuronal subtype in DS. Heterozygous loss-of-function mutations result from quantitative reduction of gene expression to 50% of normal levels. For this reason, we decided to employ a new strategy based on the activatory CRISPR-dCas9 to specifically raise the Nav1.1 protein levels by stimulating the transcription of the Scn1a gene in a DS mouse model. We demonstrated the efficiency of this system in upregulating Scn1a gene expression in cell line and primary WT neurons. ANTOLINI, LAURA, open, When studying a novel treatment with a survival time outcome, failure can be defined to include a serious adverse event (AE) among the endpoints typically considered, for instance relapse or progression. These events act as competing risks, where the occurrence of relapse as first event and the subsequent treatment change exclude the possibility of observing AE related to the treatment itself. In principle, the analysis of AE could be tackled by two different approaches: 1. the description of the observed occurrence of AE as first event: treatment ability to protect from relapse has an impact on the chance of observing AE due to the competing risks action. 2. the assessment of the treatment impact on the development of AE in patients who are relapse free in time: one should consider the occurrence of AE as if relapse would not exclude the possibility of observing AE related to the treatment itself. In the first part of the thesis we reviewed the strategy of analysis for the two approaches starting from the type of clinical question of interest. Then we identified the suitable quantities and possible estimators (crude proportion, AE rate, crude incidence, Kaplan-Meier and Aalen-Nelson smoothed estimators of the cause-specific hazard) and judge them according to two features, usually needed in a survival context: (i) the estimator should address for the presence of right censoring (ii) the theoretical quantity and estimator should be functions of time. In the second part of the thesis we proposed alternative methods, such as regression models, stratified Kaplan-Meier curves and inverse probability of censoring weighting, to relax the assumption of independence between the potential time to AE and the potential time to relapse. CASELLI, GIANFRANCO, open, At Rottapharm Biotech, a novel small molecule compound called CR13626 has emerged as a novel tyrosine kinase inhibitor with a good tropism for the brain and the ability to inhibit EGFR, VEGR2, Fyn, Yes, Lck, HGK and RET kinases relevant for the development of glioblastoma (GBM), the most common and malignant type of primary brain tumor. In addition, CR13626 resulted to be not a substrate of multidrug transporters involved in tumour resistance. Thus, the aim of my project is to characterize the activity of the compound, both in vitro and in vivo, to investigate the potential of CR13626 for glioblastoma therapy. To this purpose, I firstly investigated the ability of CR13626 to inhibit the ligand-induced activation of EGFR and VEGFR2 receptors in U87MG GBM and HUVEC-C cells, respectively, through western blot experiments. To better define the potency of CR13626 on Fyn kinase in a cellular model, I exploited the Fyn-mediated phosphorylation levels of Tau in Fyn/Tau co-transfected HEK-293 cells through a customized indirect-ELISA. Because of VEGFR2 is largely involved in promoting angiogenesis process, which contributes to tumor sustenance, I evaluated the ability of CR13626 to reduce the formation of new vessel-like structures in a HUVEC-C tube formation assay, as an indication of its antiangiogenic properties. Then I verified the effect of CR13626 on cellular proliferation in different 2D human GBM cell lines such as U87MG, U373, U87MG vIII and T98G, each harboring some of the genetic alterations/mutations present in GBM tumor cells. I also evaluated the activity of CR13626 on HEK-293 cells to assess the effect of the compound on a non-tumoral human cell line and to exclude a potential toxicity on healthy cells. open, Background: Streptococcus pneumoniae or pneumococcus, is responsible for severe invasive infections (IPD) in high risk groups and in the elderly. Moreover the pneumococcus is the most common cause of community acquired pneumonia (CAP) in adults. The aim of this study is the budget impact analysis of pneumococcal vaccination in elderly population in Italy from the perspectives of the National Health System (NHS) and Societal at 10 years of follow-up. Methods: A Markov model was developed based on the cohort of the Italian population to describe pneumococcal disease in elderly (=65 years). Three vaccination strategy scenarios were considered; No vaccination (Scenario 1), vaccination with 20% VCR (Scenario 2) and vaccination with 75% VCR (Scenario 3). The model considers costs of vaccination and other direct and indirect medical costs related to pneumococcal disease (non-bacteraemic pneumococcal pneumonia and pneumococcal meningitis). The NHS and Societal perspective and a simulated follow-up of 10 years were adopted. Results: In the base case, considering the cohort of population vaccinated followed for 10 years, the model estimated a total direct and indirect cost of €92.9, €88.8 and €77.5 million for the Scenario 1, 2 and 3. The ROI was -10% in the scenario without vaccination and 37% in the scenario with vaccination considering the impact of direct and indirect costs. Conclusions: In this study has been demonstrated that a pneumococcal vaccination in elderly populations is a strategy with a positive ROI if also indirect costs have been considered in the analysis. Metodi: È stato sviluppato un modello di Markov basato sulla coorte della popolazione italiana per descrivere il pneumococco in una coorte di anziani (=65 anni). open, Amyotrophic lateral sclerosis (ALS) is an idiopathic motor neuron disease characterized by progressive degeneration of upper and lower motor neurons, resulting in muscle atrophy, limb paralysis, and finally respiratory failure. The pathophysiology of ALS seems multifactorial but remain largely unknown, leading to limited availability of effective therapies. Therefore, there is a strong need to characterize new treatments that not only alleviate symptoms, but also improve survival and quality of life. In this research we focused on the potential therapeutic effects of three drugs belonging to the family of the growth hormone secretagogues (GHSs). The GHSs are endowed with several endocrine and extraendocrine effects that are at least in part mediated by binding to GHS-R1a, the receptor of ghrelin. In particular, we have selected three well-characterized compounds: (i) hexarelin, an agonist of GHS-R1a, which exerts cyto-protective effects at the mitochondrial level in cardiac and skeletal muscles, and could have neuroprotective effects; (ii) JMV2894, an agonist of GHS-R1a, which stimulates Ca2+ mobilization in vitro and growth hormone release in vivo, and modulates mitochondria functioning and ROS production; (iii) EP80317, an antagonist of GHS-R1a and agonist of CD36, which modulate the production of inflammatory cytokines, exerts anticonvulsant activities and has cardio-protective effects. The first objective of this study was to determine the cyto-protective capacity of hexarelin, JMV2894 and EP80317 in Neuro-2A cells subjected to oxidative stress. Neuro-2A cells were incubated for 24 h with H2O2 or with the combination of H2O2 and GHSs to monitor their effects through the quantification of (i) cell viability, (ii) NO2- release, (iii) changes in cellular morphology, (iv) apoptotic response and (v) MAPK and Akt pathways. open, Multiple Sclerosis (MS) is a chronic autoimmune disease that affects the central nervous system (CNS) leading to demyelination. The main pathological phenotypes are Relapsing-Remitting (RRMS) and Primary Progressive (PPMS) - the latter being the most severe form. Ajami B. et al. have shown that in the mouse model affected by Encephalomyelitis (EAE) - the animal model of MS- there is a strong correlation between the presence of monocytes at the level of the CNS and the worsening of the motor symptoms typical of the disease. Based on this evidence, blood samples from HC, RRMS and PPMS -all female- were collected for our study and CD14+ monocytes were isolated. Microarray experiments were conducted on these samples and subsequently qRT-PCR was performed. Bioinformatics analysis showed that RRMS distributed in two groups: one group (RR1) had similar trend to HC, the other group (RR2) had similar trend to PPMS. Gene Ontology showed that the most deregulated biological processes were those of Inflammation and Cholesterol. In addition to these patients, a second cohort of RRMS and PPMS was validated by qRT-PCR. Validation by qRT-PCR confirmed that the genes involved in cholesterol biosynthesis were deregulated in patients of both cohorts, but with specific patient-based differences. In fact, for both RRMS and PPMS, differences in expression levels of the same gene could be appreciated even among patients with the same clinical phenotype. This deregulation at the metabolic level, allowed us to hypothesize that the monocytes of these patients may have a phenotype consistent with the recently discovered Trained Immunity (TI). open, Background. Long-term implications for patients surviving SARS-CoV-2 infection, mostly for those who experienced critical COVID-19, remain to be uncovered. Objective. To measure the burden of residual disease still detectable approximately three months and one year after discharge in patients underwent to mechanical ventilation during the index hospital stay. Methods. A single-center observational study was performed by including patients discharged alive from hospital admission with main diagnosis of COVID-19, and who underwent mechanical ventilation during hospital stay. Symptoms and a wide range of instrumental signs and CT exams were assessed after approximately three months and one years from index discharge by a multidisciplinary team using standardized procedures. Results. Between March and June 2020, 203 patients were enrolled and agreed to undergo an outpatient visit after two months since discharge. About 60% patients had at least one symptom, 6.5% had low level of saturation during oximetry reading and 85.0% had at least a blood test abnormality. More than 20% patients had at least 50% of the lung parenchyma affected by abnormalities. During the visit, a specialist consultation was requested for 21.2% of the subjects. Globally, just above 20% of the patients still presented severe COVID-19 Residual Illness. Duration of hospital stay longer than 30 days was significantly associated with the risk of having severe residual illness. Conclusion. A large proportion of patients still had clinical and instrumental signs of residual illness approximately two months after discharge. Ad oggi restano ancora da scoprire le implicazioni a lungo termine per i pazienti che sopravvivono, dopo ospedalizzaizone, all'infezione da SARS-CoV-2, soprattutto per coloro che hanno contratto una forma severa di COVID-19. Obbiettivo. Misurare il carico di malattia residua ancora rilevabile a circa tre mesi e un anno dalla dimissione in pazienti sottoposti a procedure invasive di ventilazione durante la degenza indice. Materiali e metodi. È stato condotto uno studio osservazionale monocentrico includendo pazienti dimessi vivi dal ricovero ospedaliero con diagnosi principale di COVID-19 e sottoposti a procedure ventilatorie durante la degenza ospedaliera. I sintomi e un'ampia gamma di indagini strumentali ed esami TC sono stati valutati dopo circa tre mesi e un anno dalla dimissione indice da un team multidisciplinare utilizzando procedure standardizzate. Risultati. Tra marzo e giugno 2020 sono stati arruolati 203 pazienti che hanno accettato di sottoporsi a una visita ambulatoriale dopo tre mesi dalla dimissione. Circa il 60% dei pazienti aveva almeno un sintomo, il 6,5% aveva un basso livello di saturazione durante la lettura dell'ossimetria e l'85,0% aveva almeno un'anomalia dell'analisi del sangue. Migrant populations experience poor health, and their outcomes tend to be poorer in comparison with the general population. Vulnerability and inequality are further exacerbated in undocumented migrants, as the most invisible to healthcare systems. This a public health challenge requiring tailored action towards universal health coverage and health system equity. Objectives: To estimate health needs among undocumented migrants in the areas of maternal & perinatal health and COVID-19; and to test a combination of methodologies for systematic monitoring and evaluation. Methods: This research is based on three retrospective studies (cohort and cross-sectional) using a combination of diverse and complementary data sources to reflect the complex nature of health outcomes and healthcare access in undocumented migrants, including: national/regional health management information systems, third sector healthcare provider health information systems, and surveys at selected healthcare facilities. Cohort: Undocumented migrants having accessed: (i) maternity healthcare through National/Regional Health Services in Lombardy Region (Italy) from 2016 to 2020; (ii) healthcare through a third sector healthcare providers in Milan (Italy) from February 24th to May 24th, 2020; (iii) healthcare through participating healthcare providers in Switzerland (Geneva Canton), USA (Baltimore City), Italy (Lombardy Region), and France (Paris Region) from February to May 2021. Results: (i) The study on maternal and perinatal health included 1595 undocumented migrant women and their neonates. 57.37% women had ≥4 antenatal visits, 68.21% the first one within 12 weeks of gestation, 63.45% at least two ultrasound tests including one within 12 weeks of gestation, and 6.21% complete laboratory tests. AMMIRATI, ENRICO, GIANNATTASIO, CRISTINA, BACKGROUND: Identifying reliable markers associated with events after acute myocarditis (AM) is clinically relevant to planning a future follow-up. We aimed to clarify the prognostic performance of previously described cardiac magnetic resonance imaging (CMRI) markers including septal late gadolinium enhancement (LGE), versus evidence of left ventricular ejection fraction (LVEF)<50% on baseline CMRI, vs. complicated clinical presentation (CCP) of AM (defined as the presence of sustained ventricular arrhythmias [SVT] or LVEF <50% on the first echocardiogram of fulminant presentation). METHODS: We assessed 248 AM patients with onset of cardiac symptoms <30 days before admission, increased troponin, and CMRI consistent with myocarditis (median time from admission to CMRI of 6 days). The patients were retrospectively collected between February 2006 and April 2019 from 6 hospitals with a median follow-up of 1708 days (first to third quartile [Q1-Q3], 1000-2751). We assessed the prognostic performance of septal LGE vs. LVEF<50% on CMRI vs. CCP. RESULTS: The study population had a median age of 34 years (Q1-Q3: 23-41) with a male prevalence of 87.1% and a median LVEF of 61% (Q1-Q3, 55-66%) on baseline CMRI. Thirteen patients (5.2%) experienced at least one major cardiac event (including cardiac death, heart transplantation (N=1), aborted cardiac death (N=3), SVT (N=5), or heart failure hospitalization (N=5). Among these 13 patients, 10 (76.9%) had septal LGE, 8 (61.5%) had LVEF<50%, on CMRI, and 12 (92.3%) had a CCP. The best performance for these prognostic markers was the negative predictive value (NPV) ranging between 0.98 and 0.99 for CCP, while predictive value was low, ranging between 0.14 and 0.25 for LVEF<50%. CONCLUSIONS: We confirmed that the rate of major cardiac events after an AM is relatively low, and septal LGE, LVEF<50% on CMRI, and CCP are significantly associated with events. 1277, Senni Michele for the FOURIER Steering Committee and Investigators, BACKGROUND Evolocumab is a monoclonal antibody that inhibits proprotein convertase subtilisin-kexin type 9 (PCSK9) and lowers low-density lipoprotein (LDL) cholesterol levels by approximately 60%. Whether it prevents cardiovascular events is uncertain. METHODS We conducted a randomized, double-blind, placebo-controlled trial involving 27,564 patients with atherosclerotic cardiovascular disease and LDL cholesterol levels of 70 mg per deciliter (1.8 mmol per liter) or higher who were receiving statin therapy. Patients were randomly assigned to receive evolocumab (either 140 mg every 2 weeks or 420 mg monthly) or matching placebo as subcutaneous injections. The primary efficacy end point was the composite of cardiovascular death, myocardial infarction, stroke, hospitalization for unstable angina, or coronary revascularization. The key secondary efficacy end point was the composite of cardiovascular death, myocardial infarction, or stroke. The median duration of follow-up was 2.2 years. RESULTS At 48 weeks, the least-squares mean percentage reduction in LDL cholesterol levels with evolocumab, as compared with placebo, was 59%, from a median baseline value of 92 mg per deciliter (2.4 mmol per liter) to 30 mg per deciliter (0.78 mmol per liter) (P<0.001). 9, Background This study aimed at comparing, within the 2018 NIA-AA amyloidosis/tauopathy/neurodegeneration (ATN) framework, the distribution of T +/- profiles across A+patients with MCI and dementia in a retrospective, single-center, clinic-based cohort. Methods We retrospectively collected data on N=168 A+patients with either MCI due to AD (N=50) or probable AD dementia (ADD; N=118). ATN status was assigned, according to the 2018 NIA-AA framework, based on cerebrospinal fluid (CSF) biomarker concentrations. A chi(2)-test for independent samples was run to compare the distribution of A + T + vs. A + T- profiles, regardless of N status, across MCI and dementia patients. Results The most represented ATN profile in both groups was A+T+N+(MCI: 54%; dementia: 70.3%); 3.4% of dementia patients and none within the MCI cohort presented with an A+T-N+ profile. When grouping ATN profiles solely based on A and T dimensions, the prevalence of A+T+ was of 76.3% and 66% in dementia and MCI patients, respectively. No association between clinical diagnoses (i.e., MCI vs. dementia status) and AT profiles (i.e., A+T+ vs. A+T-) was detected. Discussion The distribution of A+T+ vs. A+T- does not differ between MCI and ADD, with A+T+profiles being predominant in both clinical categories. This does not support the common notion of A+T- profiles being relatively more prevalent in MCI patients, as indexing an earlier and/or less severe disease. In pulmonary hypertension (PH), both post-capillary and pre-capillary pulmonary vascular alterations may affect right ventricular (RV) afterload, eventually contributing to exercise intolerance, RV failure and prognosis. Objectives. To characterize the impact of pulmonary haemodynamics on the RV, exercise pathophysiology, and outcome in patients with PH. Methods. We firstly conducted a retrospective analysis (2007-2014) at a PH referral Center, comparing patients with PH due to left heart disease (LHD) and treatment-naïve idiopathic/heritable pulmonary arterial hypertension (PAH, n=35). Patients with PH-LHD were further subdivided according to pulmonary haemodynamics in Isolated post-capillary PH (IpcPH: diastolic pressure gradient, DPG<7 mmHg and pulmonary vascular resistance, PVR≤3 WU, n=37), Combined post- and pre-capillary PH (CpcPH: DPG≥7 and PVR>3, n=27), and “indeterminate” PH-LHD (either DPG≥7 or PVR>3, n=29). We then conducted a meta-analysis on studies focusing on the association between pulmonary haemodynamics (PVR, DPG and pulmonary arterial compliance, Ca) and outcome in PH-LHD. Results. Haemodynamic severity (pulmonary artery pressures, PVR, pulmonary vascular gradients), prevalence of echocardiographic signs of RV failure (RV dilation + dysfunction), and exercise hyperventilation linearly increased from IpcPH, to “indeterminate”, CpcPH and PAH (p<0.001), while peak oxygen consumption and prevalence of exercise oscillatory ventilation showed an opposite behavior, being higher in IpcPH. The accumulation of oxidative damage to macromolecules by RONS and ROS in BBB can be crucial in the development and progression of different CNS pathologies. In this situation, cerebral ischemia could further alter the oxidant/antioxidant balance in favour of oxidants. In this scenario, nutrition can counteract the oxidative impacts, polyphenol-enriched diets can provide beneficial effects, preventing cognitive decline and degenerative disorders. More recently, coffee has been described as a very important source of antioxidant compounds (Ricci A. et al., 2018) but its production generates large amount of waste. According to these guidelines, the aim of this study was to evaluate the antioxidant properties of several coffee-related compounds alone and combined together in an in vitro model of ischemia. The compounds used were: phytoextracts deriving from the waste of coffee production and enriched in specific polyphenolic components; and coffee metabolites found in plasma of people drinking coffee. L'accumulo di danno ossidativo alle macromolecole da parte di RONS e ROS in BEE può essere cruciale nello sviluppo e nella progressione di diverse patologie del SNC. In questa situazione l'ischemia cerebrale potrebbe alterare ulteriormente l'equilibrio ossidante/antiossidante a favore degli ossidanti. In questo scenario, l'alimentazione può contrastare gli impatti ossidativi e le diete arricchite di polifenoli possono fornire effetti benefici. Il caffè è stato descritto come una fonte molto importante di composti antiossidanti (Ricci A. et al., 2018), ma la sua lavorazione produce ogni anno grosse quantità di scarto (Mussatto et al., 2011). Seguendo queste linee guida, lo scopo di questo studio era di valutare le proprietà antiossidanti di diversi composti correlati al caffè, da soli e combinati insieme, in un modello in vitro di ischemia. I composti utilizzati sono stati: fitoestratti derivanti dagli scarti della produzione del caffè e arricchiti in specifici componenti polifenolici; e metaboliti del caffè individuati nel plasma di persone che bevono caffè. Il momento successivo alla riossigenazione provoca un aumento dei ROS, raggiungendo un picco massimo entro 1h dal ripristino delle normali condizioni di coltura (Adibhatla RM et al., 2001). Pertanto, per la valutazione delle proprietà antiossidanti dopo OGD, come condizione di maggior stress è stato scelto l'intervallo 0-1h immediatamente successivo al recupero. The first genome-wide association study in Italian ischemic stroke patients found a significant association with the missense single nucleotide polymorphism (SNP) rs4780144 in the ryanodine receptor type 3 (RyR3) gene, which leads to a potential loss of function. Multiple evidences suggested that a reduced function of RyR3 could improve stroke outcome. With this study we aimed at investigating the role of RyR3 in ischemic stroke at functional, genomic, and cellular level. Adipose-derived mesenchymal stem cells (Ad-MSCs) express RyR3 but not the other ryanodine receptors (RyR1 and RyR2). We assessed the effect of the rs4780144 genotype on intracellular calcium homeostasis in Ad-MSC lines. Our results confirmed the reduction of the RyR3 function (reduced release of calcium ions into the cytoplasm) in cells with the mutated alleles, which was statistically significant in homozygous donor. A second cohort of 319 Italian ischemic stroke patients with good clinical outcome was genotyped with Illumina Human-24 720. Genotypes of cases (both first and second cohorts) and controls were imputed using the TOPMed reference panel of human haplotypes. This second GWAS replicated the association with rs4780144 and other SNPs in the RyR3 gene. Finally, ischemic injury induced by oxygen-glucose deprivation (OGD) was evaluated in organotypic brain slices from wild-type and RyR3-knockout mice. RyR3- knockout showed a decreased susceptibility to ischemic damage compared to wild-type slices as indicated by the reduced propidium iodide incorporation and LDH release at 48 and 72 h after OGD. RyR3- knockout slices showed also a reduced swelling after OGD, indicating reduction of cytotoxic edema. However, to date no significant differences were found in gene expression analysis performed on neuronal and oxidative stress related genes. MOREO, ANTONELLA, open, The first purpose of the work was to retrospectively analyze the organizational, economic and clinical impact of performing echocardiographic examinations in the hospital ward at the patient's bed using a portable echocardiograph and a certified sonographer with centralized reading of the exams by expert echocardiographers with respect to the organizational model that requires the transport of patients to the Echocardiography Laboratory for the execution of the echocardiographic examination. The second purpose was to prospectively assess the clinical and organizational impact of the latest generation ultra-mobile ultrasound scanners that have been adopted by cardiologists as part of consulting visits to inpatient patients. METHODS Historical data on the modality and time of transport of patients to the Echocardiography Laboratory were analyzed. This model was compared in economic terms and in clinical impact with the one involving bedside examinations, recorded by the sonographer. Information was also collected such as the need for repetition and / or completion at the Laboratory, recording any discrepancies or the added value of the examination carried out at the Laboratory. For the second purpose, cardiological consultations carried out in internist departments were recorded prospectively over a period of 2 months. Subsequently, the cardiologists who carried out the cardiological consultations were equipped with an ultra-mobile ultrasound system and for 5 months were recorded the method of use, the relapse in the management of the patient and what were the clinical conditions that instead required the execution of a complete echocardiogram by the sonographer or at the Laboratory. Finally, the savings in terms of hospitalization days and the economic impact of this management for the hospital are estimated, as the cardiological consultancy could be finalized on the spot. Il secondo scopo è stato quello di valutare prospetticamente l’impatto clinico ed organizzativo di ecografi ultra-mobili di ultima generazione che sono stati adottati da parte da cardiologi nell’ambito delle visite di consulenza a pazienti degenti. METODI Sono stati analizzati dati storici sulla modalità e sui tempi di trasporto dei pazienti al Laboratorio di Ecocardiografia. Questo modello è stato confrontato in termini economici e di impatto clinico con quello che prevedeva gli esami al letto del paziente, registrati dal sonographer. Sono state raccolte anche informazioni quali la necessità di ripetizione e/o di completamento presso il Laboratorio registrando le eventuali discordanze. Per il secondo scopo sono state registrate prospetticamente nell’arco di 2 mesi tutte le consulenze cardiologiche effettuate in reparti internistici. Successivamente i cardiologi che espletavano le consulenze cardiologiche sono stati dotati di un ecografo ultra-mobile e per 5 mesi ne è stata registrata la modalità di utilizzo, la ricaduta nella gestione del paziente e quali fossero le condizioni cliniche che invece necessitavano comunque l’esecuzione di un ecocardiogramma completo da parte del sonographer o presso il Laboratorio. open, In the era of technological development, globalization and increasingly interdisciplinary work contexts, it becomes important, if not necessary, to recover and deepen those educational models that make relationships the centre of the learning process in order to train students who are able to give a sense of the tools acquired rather than being passive agents. Considering this need, this paper intends to focus on the innovation of university education in the context of Italian medical education. In order to accompany readers in the progressive approach to this theme, the articulation of the chapters proceeds from the general to the particular. Readers will begin their journey with the discovery of new learning theories associated with innovative teaching methods, to then meet the specifics of the new university education influenced by these innovations and finally the specific world of university medical education. After acquiring a knowledge of the innovative context within which the new university training in the medical context is moving, readers will be able to consult two research studies carried out within this practical-theoretical panorama. Research is presented following the structure of scientific articles. In light of the relevant literature, the objectives of the first study are: 1) to translate, adapt and validate the Conceptions of Learning and Teaching (COLT) in Italian so that it can become a tool for promoting and monitoring the implementation of a more student-centered approach in the Italian universities and 2) to acquire a first representation of the state of the art of the teaching attitude of the teachers of different Italian universities so as to be able to eventually implement specific training courses aimed at teachers. The first study shows that: 1) the Italian version of the questionnaire (COLT-IT) is composed of two sub-scales (Teacher Centredness and Appreciation of Active Leraning) each of 5 items and 2) the teachers of the Italian medi, Nell’era dello sviluppo tecnologico, della globalizzazione e di contesti lavorativi sempre più interdisciplinari diviene importante, se non necessario, recuperare e approfondire quei modelli educativi che fanno della relazione il centro del processo di apprendimento al fine di formare studenti che siano in grado di dare un senso agli strumenti acquisiti piuttosto che esserne agenti passivi. Al fine di accompagnare i lettori nel progressivo avvicinamento a questo tema, l’articolazione dei capitoli procede dal generale al particolare. I lettori inizieranno il loro viaggio con la scoperta delle nuove teorie dell’apprendimento a cui sono associati innovativi metodi didattici, per poi incontrare le specifiche della nuova formazione universitaria influenzata da queste innovazioni ed infine il particolare mondo della formazione medica universitaria. Dopo aver acquisito una conoscenza del contesto innovativo all’interno del quale si sta muovendo la nuova formazione universitaria nel contesto medico, i lettori potranno procedere alla consultazione di due ricerche svolte all’interno di questo panorama pratico-teorico. Le ricerche sono presentate seguendo la struttura di articoli scientifici così da essere auto-consistenti. open, Introduction My research activity aims to use the data from the HERCOLES study, a retrospective study on hepatocarcinoma, as an application example for the comparison of statistical methods for estimating the marginal effect of a certain treatment on standard survival endpoints (unweighted) and weighted composite endpoints. This last approach, unexplored to date, is motivated by the need to take into account the different clinical relevance of cause-specific events. In particular, death is considered the worst event but a greater relevance is also given to local recurrence compared to non-local one. To evaluate the statistical performance of these methods, two simulation protocols were developed. Methods To remove or reduce the effect of confounders (characteristics of the subject and other baseline factors that determine systematic differences between treatment groups) in order to quantify a marginal effect, it is necessary to use appropriate statistical methods, based on the Propensity Score (PS): the probability that a subject is assigned to a treatment conditional on the covariates measured at baseline. In my thesis I considered some of the PS-based methods available in literature (Austin 2013): - PS as a covariate with spline transformation - PS as a stratified categorical covariate with respect to quantiles - Pairing for PS - Inverse probability weighting (IPW) The marginal effect of the unweighted composite endpoint is measured in terms of marginal hazard ratio (HR) estimated using a Cox model. As regards the weighted composite endpoint, the estimator of the treatment effect is the non-parametric estimator of the ratio between cumulative hazards proposed by Ozga and Rauch (2019). open, Background. Healthy ageing is associated with changes in human’s body ability to modify organs and systems structure and function in response to stimuli. With this project we sought to understand whether remodelling in response to a stimulus, exercise training, altered with healthy ageing and to deepen the knowledge about running effects on the knee joint. Methods. 237 untrained healthy male and female subjects volunteering for their first-time marathon were recruited. At baseline and after 6 months of unsupervised training, race completers underwent tests including 1.5T cardiac magnetic resonance, brachial and non-invasive central blood pressure (BP) assessment and a 3.0T bilateral knee magnetic resonance. Biological “aortic age” was calculated from the baseline chronological age-stiffness relationship. Change in stiffness was assessed at the ascending (Ao-A) and descending aorta at the pulmonary artery bifurcation (Ao-P) and diaphragm (Ao-D). For analysis, runners were divided by age (O35: ≥35y.o.; U35:  34y.o.) Results. Injury and completion rates were similar among groups. 138 runners (under 35 [U35]: n=71, females=49%; over 35 [O35]: n=67, females=51%) completed the race. On average, U35 were faster by 37 minutes (12%). Training induced a small increase in left ventricle (LV) mass in both groups (3g/m2, p<0.001), but U35 also increased ventricular cavity sizes (LV end-diastolic volume [EDV]i +3%; LV end-systolic volume [ESV]i +8%; right ventricle [RV] EDVi +4%, RVESVi +5%; p<0.01 for all). Systemic aortic compliance fell in the whole sample by 7% (p=0.020) and, especially in O35, also systemic vascular resistance (-4% in the whole sample, p=0.04) and blood pressure (systolic/diastolic, whole sample: brachial -4/-3 mmHg, central -4/-2 mmHg, all p <0.001; O35: brachial -6/-3 mmHg, central -6/-4 mmHg, all p<0.001). PESCE, MAURIZIO, open, Despite the preferred application of arterial conduits, the greater saphenous vein (SV) remains indispensable for coronary artery bypass grafting (CABG), especially in multi-vessel coronary artery disease. Early remodeling induced by altered wall mechanics has been recognized to play a key role in SV graft disease. The mechanism remains, however, unknown. The aim of this work was to unveil the existence of a mechanical effect in SV graft failure, due to changes in the hemodynamic conditions occurring in SV grafts after transplantation into coronary position. SV segments from patients receiving coronary artery bypass grafts were stimulated in a coronary ‘pulse-duplicator’ bioreactor with either CABG or venous hemodynamic conditions. Human saphenous vein derived-smooth muscle cells (SMCs) and adventitial progenitors (SVPs) were mechanically stretched in vitro using Flexcell Tension System. The pro-pathological differentiation of resident cells in the vein, resulting from exposure to coronary flow, derives from a mechano-perception phenomenon. In particular, adventitial cells are activated by the release of Thrombospondin-1 by stretched smooth muscle cells in the veins exposed to coronary flow, thus suggesting a connection between the mechanical stress experienced by the vascular wall and the remodeling of the bypass. In addition, a gene enrichment analysis of transcripts up/downmodulated by mechanical stress revealed the involvement of a mechanosensing transcriptional circuitry (HIPPO/YAP/TEAD pathway) in cells stimulated with the cyclic strain. These findings suggest the existence of a ‘mechano-paracrine’ effect due to CABG-specific wall strain in SV grafts. In particolare, le cellule avventiziali vengono attivate a seguito del rilascio di Trombospondina-1 da parte delle cellule muscolari lisce nelle vene esposte al flusso coronarico, suggerendo così una connessione tra lo stress meccanico sperimentato dalla parete vascolare e il rimodellamento del bypass. CERRITO, MARIA GRAZIA, open, Triple-negative breast cancer (TNBC) represents the most malignant subtype of breast cancer, often associated with short disease-free survival. The maximum tolerated dose, used as standard-of-care chemotherapy (STD), remains the primary therapeutic option for TNBC patients, even though it fails to block distant recurrences, which is the first cause of death. Recently, it has been tested a different therapeutic approach, called metronomic chemotherapy (mCHT), where the minimum biological effective dose of the chemotherapeutic agent is given as a continuous administration without drug-free breaks. So far, the results show a strong stabilization of cancer growth and improved cancer patients’ quality of life. mCHT has direct cytotoxic effects on tumors and inhibits endothelial cell proliferation. In this thesis we investigated, for the first time, the effect of the mCHT administration of 5-Fluorouracil (5-FU) plus Vinorelbine (VNR) in endothelial and TNBC cell lines in comparison to the STD treatment. In particular, it has been shown that the mCHT administration of 5-FU and VNR both in single and in combination affects endothelial and TNBC cells at doses significantly lower than the STD one. Despite the low doses used, mCHT 5-FU+VNR is more effective than the STD in inhibiting cell migration and colony formation of both endothelial and TNBC cells. To simulate TNBC and endothelial cells' crosstalk, we utilized an indirect co-culture, which showed that medium conditioned by TNBC cells treated with mCHT 5-FU+VNR completely blocks endothelial cells migration and colony formation. We also pointed out that the modality of cell death of endothelial and TNBC cells depends on the schedule of treatment. Recentemente, è stato testato un diverso approccio terapeutico, chiamato chemioterapia metronomica (mCHT), che prevede la continua somministrazione della minima dose biologicamente attiva senza pause libere da farmaco. Ad oggi, i risultati mostrano una forte stabilizzazione della crescita del cancro ed un miglioramento della qualità della vita delle pazienti. La chemioterapia metronomica ha un effetto sia antitumorale che anti-angiogenico. In questa tesi abbiamo indagato, per la prima volta, l’effetto della somministrazione metronomica di 5-Fluorouracile (5-FU) e Vinorelbina (VNR) su linee cellulari di TNBC ed endoteliali, in confronto alla somministrazione STD. In particolare, abbiamo dimostrato che la somministrazione metronomica di 5-FU e VNR, sia in singolo che in combinazione, colpisce le cellule endoteliali e di TNBC a dosi significativamente inferiori della terapia STD. Nonostante le bassi dosi utilizzate, la somministrazione metronomica di 5-FU+VNR è più efficiente della STD nell’inibire la migrazione cellulare e la formazione di colonie sia delle cellule endoteliali che delle cellule TNBC. Per simulare l’interazione tra cellule endoteliali e TNBC, abbiamo utilizzato una co-coltura indiretta. GENOVESE, PIETRO, open, The scope of genome engineering in hematopoietic stem/progenitor cells (HSPCs) has broadened from random to precise genome insertions for treating genetic diseases of the blood lineages. Targeted editing of inherited mutant genes allows in situ correction and functional reconstitution with preserved expression control. We recently showed that both the induced double-strand DNA breaks and the AAV6 genome trigger a p53-dependent DNA damage response in HSPC delaying proliferation and decreasing hematopoietic reconstitution after xenotransplantation. Suppression of this response by transient expression of a dominant negative p53 released cell-cycle block and rescued hematopoietic reconstitution. Yet, the underlying biology remained unknown as well as the impact of gene editing on clonal dynamics of HDR-edited HSPC upon transplantation. Moreover, it has long been contended that the quiescence of primitive HSC constrains HDR-mediated gene editing, thus limiting its perspective clinical applications in several diseases. Here, we first overcame such constraints by transiently expressing the adenovirus 5 protein E4orf6/7, which operates the major cell cycle controller E2F, together with the nuclease. By global and targeted gene expression analysis we showed engagement of targeted cells in S/G2 phases with concomitant upregulation of all major components of the HDR machinery, thus increasing the efficiency of targeted transgene insertion. Combined E4orf6/7 expression and p53 inhibition enhanced >50% HDR efficiency within human graft surpassing the levels reported until now in the literature. Such outcome was reproducible across several HSPC donors and sources, genomic loci and conceivably portable to most types of editing platforms. open, The high prevalence of metabolic and cognitive disorders represents one of the major issues of health systems. Recent findings indicate that obesity may affect brain functions however the effect of a high-fat diet (HFD) on the central nervous system is not fully understood. The aim of this study is the evaluation of the influence of HFD on neuroinflammation and regional brain function in a mouse model of Insulin Resistance (IR) focusing on sex-dependent differences induced by peripheral metabolic impairment. C57BL/6J male and female mice were fed with standard chow or HFD (45%/60%) for 35 weeks. Animals were monitored weekly for body weight, haemato-chemical analysis (circulating glucose, total cholesterol, ALT, and AST), and glucose tolerance test (GTT). Positron Emission Tomography (PET) imaging studies were performed longitudinally using [18F]-FDG and [18F]-VC701 as radiotracers, to measure respectively glucose consumption and microglia/macrophages activation within the brain. Magnetic Resonance Imaging (MRI) and Magnetic Resonance Spectroscopy (MRS) was used to measure the effects of diet on hepatic lipid content and peri abdominal fat accumulation. Finally, post-mortem transcriptome analysis was applied to the anterior cortex to reveal potential modification in gene expression. HFD induced a significant increase in body weight, glucose tolerance, and haemato-chemical parameter in a sex dependent manner. HFD diet increased peri abdominal fat accumulation in male and lipid content in the liver as revealed at MR spectroscopy. This last effect was particularly evident in females. PET [18F]-FDG showed a relative increase in glucose metabolism in the anterior region of the brain including the olfactory bulbs in both male and female mice while an increase in the anterior cortex was found in males mice fed with 60% HFD. 5203, Background: Surgery is the main modality of cure for solid cancers and was prioritised to continue during COVID-19 outbreaks. This study aimed to identify immediate areas for system strengthening by comparing the delivery of elective cancer surgery during the COVID-19 pandemic in periods of lockdown versus light restriction. Methods: This international, prospective, cohort study enrolled 20 006 adult (≥18 years) patients from 466 hospitals in 61 countries with 15 cancer types, who had a decision for curative surgery during the COVID-19 pandemic and were followed up until the point of surgery or cessation of follow-up (Aug 31, 2020). Average national Oxford COVID-19 Stringency Index scores were calculated to define the government response to COVID-19 for each patient for the period they awaited surgery, and classified into light restrictions (index <20), moderate lockdowns (20-60), and full lockdowns (>60). The primary outcome was the non-operation rate (defined as the proportion of patients who did not undergo planned surgery). Cox proportional-hazards regression models were used to explore the associations between lockdowns and non-operation. Intervals from diagnosis to surgery were compared across COVID-19 government response index groups. This study was registered at ClinicalTrials.gov, NCT04384926. Findings: Of eligible patients awaiting surgery, 2003 (10·0%) of 20 006 did not receive surgery after a median follow-up of 23 weeks (IQR 16-30), all of whom had a COVID-19-related reason given for non-operation. Light restrictions were associated with a 0·6% non-operation rate (26 of 4521), moderate lockdowns with a 5·5% rate (201 of 3646; adjusted hazard ratio [HR] 0·81, 95% CI 0·77-0·84; p<0·0001), and full lockdowns with a 15·0% rate (1775 of 11 827; HR 0·51, 0·50-0·53; p<0·0001). 5549, Background: This study aimed to determine the impact of pulmonary complications on death after surgery both before and during the severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) pandemic. Methods: This was a patient-level, comparative analysis of two, international prospective cohort studies: one before the pandemic (January-October 2019) and the second during the SARS-CoV-2 pandemic (local emergence of COVID-19 up to 19 April 2020). Both included patients undergoing elective resection of an intra-abdominal cancer with curative intent across five surgical oncology disciplines. Patient selection and rates of 30-day postoperative pulmonary complications were compared. The primary outcome was 30-day postoperative mortality. Mediation analysis using a natural-effects model was used to estimate the proportion of deaths during the pandemic attributable to SARS-CoV-2 infection. Results: This study included 7402 patients from 50 countries; 3031 (40.9 per cent) underwent surgery before and 4371 (59.1 per cent) during the pandemic. Overall, 4.3 per cent (187 of 4371) developed postoperative SARS-CoV-2 in the pandemic cohort. The pulmonary complication rate was similar (7.1 per cent (216 of 3031) versus 6.3 per cent (274 of 4371); P = 0.158) but the mortality rate was significantly higher (0.7 per cent (20 of 3031) versus 2.0 per cent (87 of 4371); P < 0.001) among patients who had surgery during the pandemic. The adjusted odds of death were higher during than before the pandemic (odds ratio (OR) 2.72, 95 per cent c.i. 1.58 to 4.67; P < 0.001). In mediation analysis, 54.8 per cent of excess postoperative deaths during the pandemic were estimated to be attributable to SARS-CoV-2 (OR 1.73, 1.40 to 2.13; P < 0.001). Conclusion: Although providers may have selected patients with a lower risk profile for surgery during the pandemic, this did not mitigate the likelihood of death through SARS-CoV-2 infection. Bioinformatics analyses raw experimental data with the aim to obtain information describing biological processes, thus providing a powerful tool to investigate specific molecular and genetic mechanisms. This domain knowledge in combination with genomics allows to decipher the interrelationships between genes, regulatory elements, metabolic pathways, and protein interactions. Deep learning, a subdiscipline of machine learning, has been recently applied to the field of genomics, leading to remarkable results. The two main objectives of this study were: the development and application of bioinformatic tools for the study of the genetic basis of acute lymphoblastic leukaemia, and the usage of deep learning techniques for the identification of small non-coding RNA elements in the human genome. GENOVESE, PIETRO, GRANUCCI, FRANCESCA, open, Background The X-linked hyper-IgM syndrome type I (HIGM1) is caused by inactivating mutations in the CD40 ligand gene (CD40LG) that disrupt the T cell helper function on B cells and macrophages. This disease represents an ideal candidate for a gene correction strategy because preclinical studies of Hematopoietic Stem Cell (HSC) gene therapy have already shown i) evidence of potential efficacy even with few amounts of transduced cells; ii) critical safety issues due to unregulated transgene expression. Since in HIGM1 the genetic defect is not lethal to T cells, we aim to apply our gene editing strategy on autologous T cells that could be used to provide immediate therapeutic benefit to the patients by resolving pre-existing infections prior to a definitive HSPC transplant. Methods To establish which are the therapeutic threshold levels and transplant conditions required to achieve immune reconstitution and functional immunologic restoration with corrected cells, we infused different doses of WT T cells into HIGM1 mice pre-conditioned or not with different lymphodepleting regimens and performed competitive transplants of WT and Cd40lg-/- HSPC in the mouse model. Results While longitudinal blood analyses showed a long-term, stable T cell engraftment in all the conditions, highest engraftment rates were obtained in mice transplanted after chemotherapy treatment with cyclophosphamide (CPA). All the transplanted mice showed a partial rescue of the antigen-specific IgG response after immunization with Keyhole Limpet Hemocyanin (TNP-KLH) but a higher rescue was observed in mice pre-conditioned with CPA. These mice also showed the presence of TNP-KLH specific IgG producing B cells and germinal centers within splenic lymphoid follicles. HIGM1 mice reconstituted with increasing proportions of WT HSPC displayed a dose-dependent rescue of the T cell mediated immune response. TIRANTI, VALERIA, open, Mitochondrial diseases (MD) are a clinically heterogeneous group of disorders that arise as a result of dysfunction of the mitochondrial respiratory chain (RC) and oxidative phosphorylation (OXPHOS). Mitochondrial functions are under the control of two different genomes: mitochondrial DNA (mtDNA) and nuclear genome (nDNA). Childhood phenotypes are often associated with nDNA mutations; in recent years, new-generation sequencing technologies (Next Generation Sequencing-NGS) have identified novel causative genes; in collaboration with other centers we contributed to the definition of phenotype associated with the new identified disease genes. The application of this technique has also been extended to the study of mtDNA: even if more than 100 mutations and deletions in mtDNA have been described in association with an extremely heterogeneous spectrum of clinical presentations, only a few of them are associated with well-defined clinical syndromes in childhood. We performed a systematic evaluation of clinical, instrumental, metabolic and biochemical data of a large cohort of patients affected by the most common MD in childhood: Leigh syndrome. We analyzed in this population, genotype-phenotype correlation in nDNA and mtDNA gene associated cases in order to identify diagnostic clues for mtDNA related Leigh syndrome. In genetically unresolved cases and various phenotypes (Leigh syndrome, leukodystropy..), we performed mtDNA screening using next-generation sequencing (NGS) technologies in order to assess, with high accuracy, point mutations and single or multiple large deletions, both in homoplasmic or heteroplasmic state. We identified both novel and known mutations associated to unexpected phenotype (i.e. CO3 gene). Le funzioni mitocondriali sono sotto il controllo di due diversi genomi: DNA mitocondriale (mtDNA) e genoma nucleare (nDNA). Le forme infantili sono più spesso associate a mutazioni nel nDNA; negli ultimi anni tecnologie di sequenziamento di nuova generazione (Next Generation Sequencing-NGS) hanno permesso di identificare nuovi geni malattia; in collaborazione con altri centri abbiamo contribuito alla definizione del fenotipo associato ai nuovi geni malattia identificati. A diet containing high amounts of plant-based foods has been associated with a reduced risk of cardiovascular diseases and the beneficial effect has been attributed to the antioxidants found in the foods. However, findings from randomized controlled trials on the role of antioxidant supplementation have been disappointing, reporting null results or even harmful effects. It has been suggested that antioxidants interact with each other to promote cardiovascular health. Therefore, the Non Enzymatic Antioxidant Capacity (NEAC) assay has been proposed, which measures the antioxidant potential of different dietary sources considering interactions between them. This thesis aimed to further clarify the effect of dietary antioxidants on the risk of cardiovascular diseases, with particular interest in measuring NEAC from diet. The specific aims were to prospectively study whether dietary NEAC is associated with a lower risk of myocardial infarction, stroke and heart failure in subjects free from CVD or cancer. Four studies were conducted using data from two large Swedish cohorts. Multivariable Cox proportional hazard regression models were fitted to estimate hazard ratios (HRs) with 95% confidence intervals (CIs). Along with a better understanding of these pathologies, there is a need for improved risk detection, for determination of prognosis and for improved and personalized treatment. Therefore, it is important to develop and implement new approaches, such as proteomic analysis of urine- based liquid biopsy. Indeed, the use of proteome analysis for the discovery of clinically relevant proteins, known as clinical proteomics, for example for the identification of earlier and prognostic markers may actually foster the translation of basic discoveries into clinical applications for the benefit of the patient. Easily accessible biological sample, such as urine is valuable sources of biomarkers for pathologies related to kidney. In fact, urine can be collected in large quantities and in non-invasive way; additionally, it is less complex than other bodily fluids. Unfortunately, in diverse cases, this information is often technically difficult to be mined because the disease-related proteins are often present in very low concentrations, are frequently labile, and are hidden by high-abundance proteins such as albumin or uromodulin. In this context, the proteomic study of the urinary extracellular vesicles (UEv) represents a valid alternative to reveal and discovery these hidden molecular landscape. UEv are nanometer-sized vesicles (>150 nm), that can originate from endothelial cells, podocytes or tubular epithelial cells. Their molecular composition depends upon the type, and even tatus, of the producer cell. GBM is an aggressive solid tumor that mainly affects white matter and the corpus callosum. open, Background: Uric acid (UA) has been related to in-hospital mortality in ACS patients. Furthermore, it has been related to early relapse of non-fatal cardiovascular events and to intermediate outcome such as use of intra-aortic balloon pump, noninvasive ventilation, longer inward stay, bleeding but also clinical presentation with AF or heart failure. Aim of the study: principal aim of our study was to evaluate the role of UA as a possible determinants of in-hospital mortality (primary outcome) and in hospital complications (secondary outcomes). Secondary aim was to identify the best cut-off and to evaluate diagnostic performance of already used cut-off (the classic one of > 6 mg/dL in female and 7 mg/dL in males, and a recently described one with 5.26 mg/dL in females and 5.49 mg/dL in males). Methods: we analyze data of 563 patients admitted for ACS at the Cardiological Intensive Care Unit of the Niguarda Ca’ Granda Hospital. We consider as outcome in-hospital mortality, inward myocardial infarction, instent thrombosys, bleeding, stroke, clinical presentation with heart failure of AF, inotropes, intra-aortic balloon pump and non-invasive ventilation uses during hospital stay, three vessels coronaric involvement at the coronary angiogram and EF both at admission and at discharge. Results: mean age was 66.5 ± 12.3 years, 79.2% of the patients were males and 49.9% of the ACS were STEMI. GROSSMAN, MURRAY, open, Recent studies of Alzheimer’s disease (AD) spread suggest that pathology may be transmitted from one brain area to another either via local diffusion or long-way transport via white matter pathways. However, this hypothesis requires more confirmations, and it’s even more unclear whether such models are applicable in non-amnestic AD (naAD), a group of AD phenotypes characterized by relative spared episodic memory at onset and domain-specific cognitive impairments. Few studies to date have in fact addressed the longitudinal spread of disease in naAD, and all of them considering no more than two variants. At first we compared 240 T1-weighted anatomical MRIs from 129 AD patients with elderly controls’ scans to assess atrophy in each of 120 regions-of-interest (ROIs); then we computed disease progression models separately for each phenotype: typical amnestic AD (aAD), logopenic variant primary progressive aphasia (lvPPA), posterior cortical atrophy (PCA), corticobasal syndrome (CBS), and frontal-variant AD (fvAD). All patients had autopsy or cerebrospinal fluid (CSF) evidence of AD pathology. Results from the amnestic cohort were used to determine appropriate parameters for the phase assignment algorithm, based on association with Braak pathology staging. For each AD variant, 4 phases of regional atrophy were defined based on decreasing frequency of atrophy across participants. We observed a unique distribution of accumulating atrophy for each phenotype. Phase 1 ROIs in our model represent the anatomical origin for each phenotype, including: medial temporal lobe (MTL) for the aAD group (spared in the other phenotypes), left lateral temporal lobe for lvPPA, occipito-parietal cortex for PCA, temporo-parietal cortex for CBS, and fronto-temporal cortex for fvAD. open, The number of acute myeloid leukemia (AML) patients cured through allogeneic hematopoietic cell transplantation (allo-HCT) is constantly increasing. The therapeutic effectiveness of this procedure mainly relies on the transfer from the donor to the patient of immune cells, capable of recognizing and eliminating residual tumor cells. Still, up to 50% of transplanted AML patients will eventually relapse, and the prognosis of these patients remains extremely poor. Thus, aim of my thesis work was to improve current understanding on the immunobiology of post-transplantation relapse, by investigating i) how therapies and leukemia itself affect immune reconstitution, ii) how to refine detection of leukemia reappearance at the stage minimal residual disease (MRD), and iii) how to uncover the molecular mechanisms at the basis of leukemia immune evasion. In particular, I will first present the results of a prospective study aiming at evaluating the clinical utility of monitoring patient-specific chimerism on peripheral blood, instead of the currently used bone marrow specimens, employing quantitative PCR (qPCR) for the early detection of leukemia relapses after transplantation. Will next present the results of two studies on the dynamics of recovery of NK and T cells after allo-HCT. Both studies aim at understanding the determinants of donor immune system failure in controlling AML disease recurrence with the potential implications of using the identified features as biomarkers to predict post-transplantation relapse. In the last sections I will present both published and unpublished data on the biological mechanisms underlying post-transplantation disease relapse, reporting how this knowledge can be easily translated in novel therapeutic rationales to combat disease recurrence. open, The actual gold standard to exclude the malignant nature of thyroid nodules in the clinical routine is represented by thyroid Fine Needle Aspirations (FNAs) biopsies. Thyroid FNAs are safe and cost-effective. Approximately the 20-30% of cases have an indeterminate for malignancy final report. These patients undergo diagnostic (and not therapeutic) thyroidectomy, but after surgery the 80% of these thyroid nodules are benign. This overtreatment has of course important consequences in the quality of life of the patients and high healthcare costs. The application of -omics techniques might have a potential role in the research for new diagnostic markers able to discriminate benign from malignant nodules, thus minimizing the challenging cases of indeterminate for malignancy. Mass spectrometry is one of the most important analytical tools able to obtain information regarding the molecular composition of a sample, the presence of biomolecules and their abundance. Due to the promising results recently obtained with MALDI-MSI in the identification of proteomic signals able to differentiate between benign and malignant cases from the analysis of thyroid tissue after surgery , the idea was to apply for the first time MALDI-MSI on real thyroid FNAs biopsies. open, Multiple sclerosis (MS) is the prototype of demyelinating diseases, in which both gray and white matter (GM/WM) pathology contribute to impairment of several cognitive domains including attention, mental processing speed, memory, executive and visuospatial functions, as well as many aspects of social cognition. Such deficits have been reported in all stages and subtypes of the disease, and result in significant, negative consequences for mood and quality of life of people with MS. The main goal of the current magnetic resonance imaging (MRI) study was to investigate the effect of MS on cognition and brain structure, by combining neuropsychological and morphological investigations. More precisely, we aimed to analyze changes of main cognitive functions over time in mild and early relapsing-remitting MS (RRMS) outpatients compared to healthy subjects, and correlated these findings to GM regional volume changes. We were also interested to explore the impact of MS on many aspects of social cognition, mood, fatigue, psychological well-being, and quality of life of these patients between the beginning and the end of the study. The first important MRI result was the identification of a right temporal atrophy pattern (inferior temporal gyrus and middle temporal pole) in the RRMS group compared to normal controls, which was unchanged between the baseline and follow-up. After one year, a considerable atrophy in deep GM of right hemisphere (amygdala, globus pallidus and putamen) and cerebellum (14.2%) emerged, while disappeared in the left putamen and insula. open, The burden of a condition, illness or risk factor on the population is a fundamental issue in Public Health and evaluating this burden appears to be necessary to improve health systems and policies, and to help decision makers for a better allocation of public resources. The population attributable fraction, a measure used to assess the burden of disease, allows to determine the number of disease’s cases that would be avoided or prevented if a risk factor was eliminated. Clinical Pathways are composed by several evidence-based healthcare intervention and are considered both as tool for patients’ care and as a way to describe the structure of a care process that is worldwide used to make care processes transparent and to improve the efficiency and quality of health care. However, the real impact of these pathways on several clinical outcomes, in particular in outpatient settings, is a little explored field. If we consider the subject’s adherence to recommendations contained in Clinical Pathways as a ‘gold standard’ of healthcare to avoid adverse clinical events/complications, the condition of ‘non-adherence’ represents a risk factor that could be controlled and limited, if not even removed. The aim of this thesis is to assess the impact of healthcare interventions for specific health conditions, from the Public Health point of view, in terms of (i) size of the problem, (ii) process of care and (iii) association between process and outcomes. To this purpose, my thesis is composed of four parts that lead to explore the main issue of this work and to understand how we can measure the impact of some healthcare interventions on subjects affected by several chronic conditions. After an introduction to the concept of Burden of Disease and its possible application in the assessment of impact of healthcare interventions, Clinical Pathways and their current use are presented. open, Background: Subjects with mental disorders have a higher risk of suicide behaviors than the general population. So, to date, researchers have investigated some biomarkers possibly related to suicidality. Despite many studies have reported a possible relationship between low lipid serum levels and suicide attempt, conflicting results have emerged. Aim We investigated whether serum total cholesterol, LDL cholesterol and triglycerides are associated with recent suicide attempts in subjects with different mental disorders. Methods We conducted a cross-sectional study, including 593 consecutively admitted inpatients with schizophrenia spectrum, bipolar, major depressive, and personality disorders. Serum lipid levels were compared between subjects admitted for a recent suicide attempt and those without such recent history. Moreover, according to hypothesis that links impulsivity and violence with low serum lipid levels, the association between lipid levels and violent suicide attempt was assessed. Results We did not find any association of total and LDL cholesterol and triglycerides with suicide attempts, also considering diagnosis and suicide methods. In addition, a post-hoc analysis showed a trend toward significance (p=0.06) in the association between high cholesterol level (≥160mg/dL) and recent suicide attempt. Conclusions Our results do not support the hypothesis of association between lipid profiles and suicide attempts in subjects with different mental disorders. GRASSILLI, EMANUELA, open, Lung cancer is the leading cause of cancer-related death worldwide both in men and women. In particular, Non-Small Cell Lung Cancer (NSCLC) is the most common subtype and is relatively insensitive to chemotherapy and radiation therapy. Despite the development of personalized molecular targeted therapies against specific driver mutations and the combination of chemotherapy with targeted therapy, lung cancer mortality remains very high because of intrinsic and acquired resistance. Thus, new strategies to overcome these limitations are needed. Recently, we discovered a new isoform of the Bruton Tyrosine Kinase (BTK), referred as p65BTK. We characterized p65BTK as a novel oncogene and a pivotal downstream effector of RAS. Moreover, we showed that its inhibition affected growth and survival of colon cancer cells and reverted resistance to chemotherapy. The aims of this project were: study the role of p65BTK in NSCLC cell biology and verify whether p65BTK may be a novel theranostic target in NSCLC. Studying a cohort of 382 patients, we observed that p65BTK was expressed in 50% of NSCLC patients’ tumors and its expression correlated with histotype, smoke habit and EGFR mutational status. In particular, we found p65BTK significantly more expressed in adenocarcinoma than in squamous carcinoma histotype and in non-smoker as compared to smoker patients. Moreover, its expression was significantly higher in non-smoker patients bearing wild type EGFR. Notably, these patients are not eligible for treatments with EGFR inhibitors due to a lack of EGFR mutation. By western blot analysis we confirmed p65BTK overexpression both in vitro, in NSCLC human cell lines with mutations in RAS or in one of the components of the RAS/MAPK pathway, and ex vivo, in tumor-derived primary cells from Kras/Trp53 null mice, suggesting that p65BTK overexpression correlate with a hyper-activated RAS/MAPK pathway also in NSCLC. open, Mucopolysaccharidosis type I (MPS-I) is a lysosomal storage disorder due to deficiency in the alpha-L-iduronidase enzyme, resulting in the progressive accumulation of glycosaminoglycans (GAGs) in multiple organs and the consequent multiorgan dysfunction. Hematopoietic stem cell transplantation (HSCT) and enzyme replacement therapy (ERT) are the currently available therapies for MPS-I patients. Despite their effectiveness in correcting most clinical manifestations, muscoloskeletal abnormalities and neurocognitive defects still impact quality of life of treated patients. It is widely accepted that an early intervention may be crucial to obtain a better outcome. On the basis of these observations, we tested the therapeutic efficacy of HSCT and ERT, alone or in combination, administered at birth, in a MPS-I mouse model. We demonstrated that all three treatments were able to prevent biochemical and pathological manifestations of the disease in visceral organs, and, in particular, the combination therapy had a better outcome in difficult-to-treat organs, such as kidney and heart. Of note, transplant procedure can halt the production of antibodies specific for the recombinant enzyme that could reduce ERT efficacy. Regarding the severe skeletal disease, we observed a differential effect in reduction of bone disease, with a significant better correction in transplanted mice, in the presence of ERT or not. Otherwise in the brain, all the treatments provided a moderate decrease of inflammation without an evident metabolic correction.
