3. Therapeutic effect of Ixekizumab combined with hemoperfusion in a patient with severe psoriasis (依奇珠单抗联合血液灌流治疗重度银屑病1例疗效观察)

In this paper, clinical data of a case of hemodialysis patients with erythrodermic psoriasis who were treated with Ixekizumab injection combined with hemoperfusion were retrospectively analyzed. The first administration was subcutaneous injection of Ixekizumab 80 mg, followed by injection every three weeks (6 doses in total), and maintenance therapy with 80 mg once a month. The hemodialysis treatment was performed three times a week, and hemoperfusion treatment was carried out every two weeks. After more than 1 year of observation, the patient's current systemic skin lesions were significantly improved, without scaling or skin itching and other manifestations. Ixekizumab combined with hemoperfusion had achieved good clinical efficacy in the treatment of erythrodermic psoriasis. (本文回顾1例红皮病型银屑病血液透析患者临床资料, 使用依奇珠单抗注射联合血液灌流治疗。依奇珠单抗方案: 第一次皮下注射依奇珠单抗注射液80 mg, 此后每3周注射1次(共6次), 并以80 mg每月1次维持治疗; 透析方案: 血液透析治疗每周3次, 每2周进行1次血液灌流治疗。经1年多的观察, 患者全身皮损明显改善, 无鳞屑及皮肤瘙痒等表现, 提示依奇珠单抗联合血液灌流治疗红皮病型银屑病取得很好的临床疗效。)

目的探讨抗糖药物卡格列净(CGLZ)对阿霉素所致肾病综合征(NS)模型大鼠的治疗作用,以及超声造影(CEUS)联合彩色多普勒血流显像(CDFI)在治疗过程中的评价作用。方法56 只SD 雄性大鼠随机分为正常组(NG)、模型组(MG)、泼尼松(PAT)组(PG)、小剂量CGLZ 组(LSCG)、大剂量CGLZ 组(HSCG)、小剂量CGLZ + PAT 组(LUCG)和大剂量CGLZ +PAT 组(HUCG),每组8 只。采用阿霉素2 次尾静脉注射法诱导制备NS 大鼠模型,以常规PAT 治疗为参照予以每日药物灌胃处理,连续6周。在灌胃前1 d和灌胃2、4、6周末分别行24h-UTP 定量检测,灌胃6周末行右肾动脉CDFI 及右肾CEUS 检查,次日取腹主动脉血行血清学检测。结果与NG 大鼠的检测指标相比,MG 大鼠的24 h-UTP 升高(P<0.01),血清ALB 降低,TG、TC、LDL 升高(P<0.01),CDFI 显示RRCT 变薄(P<0.01),肾动脉血流指标RA-PI、RA-RI、RA-S/D 升高(P<0.05),CEUS 图像显示TIC 曲线参数TTP、AT、AUC 升高,DPI、A 下降(P<0.01)。药物治疗后,与MG 大鼠的检测指标相比,24 h-UTP 检测结果显示2周后仅LSCG 组下降(P<0.01),6周后各药物组均明显下降(P<0.01);血清学检测结果显示6周后所有CGLZ 治疗组大鼠的血清ALB 升高(P<0.05),LSCG 大鼠的TG 下降(P<0.01),LUCG 大鼠的TC、LDL 下降(P<0.05);CDFI 检查显示所有CGLZ 治疗组大鼠肾脏的RRCT 变薄程度减轻(P<0.01),LSCG 大鼠的RA-PI、PG 大鼠的RA-RI 以及PG、LSCG、HSCG、LUCG 大鼠的RA-S/D 均降低(P<0.05);CEUS 检查显示所有药物治疗组大鼠肾脏TIC 曲线的TTP、AT、AUC降低(P<0.01),PG、HSCG、LUCG、HUCG 大鼠的DPI 升高(P<0.01)。结论CGLZ 具有治疗NS 作用,以小剂量用药为最佳。CEUS 联合CDFI 检查可用来评估NS 模型大鼠及其药物治疗前后的肾脏形态及血流动力学变化,有助于指导临床应用。

为了实现羊骨副产物的有效利用和为羊骨油高值化产品开发提供基础数据,以羊腿骨为原料,提取其中的油脂,分别采用高效液相色谱法(HPLC)、多维质谱鸟枪法(MDMS-SL)、气相色谱-质谱联用法(GC-MS)对羊骨油的甘油酯组成、脂质组学、脂肪酸组成进行分析。结果表明:羊骨油中甘油三酯含量达到96.76%,还有少量的甘油二酯(1.41%),而甘油一酯未检出;羊骨油中共鉴定出19种酰基肉碱、17种鞘磷脂、37种磷脂酰胆碱、39种甘油三酯,总含量分别为5.76、96.58、85.18 nmol/g和15.76 μmol/g;羊骨油中共鉴定出11种脂肪酸,其中饱和脂肪酸含量为11.51%, 单不饱和脂肪酸含量为72.34%,多不饱和脂肪酸含量为7.56%。 In order to realize the effective utilization of by-products sheep bones and provide basic data for the development of high-value sheep bone oil products, sheep leg bones were used as raw materials to extract oil, and the glyceride composition, lipidomics and fatty acid composition of sheep bone oil were analyzed by high performance liquid chromatography (HPLC), multidimensional mass spectrometry shotgun method (MDMS-SL) and gas chromatography-mass spectrometry (GC-MS) respectively. The results showed that the content of triglycerides in sheep bone oil reached 96.76%, and a small amount of diglycerides (1.41%) was identified, while no monoglycerides was detected. A total of 19 acylcarnitines, 17 sphingomyelins, 37 phosphatidylcholines and 39 triglycerides were identified in sheep bone oil, with total contents of 5.76, 96.58, 85.18 nmol/g and 15.76 μmol/g, respectively. 11 kinds of fatty acids including 11.51% saturated fatty acids, 72.34% monounsaturated fatty acids and 7.56% polyunsaturated fatty acids were detected in sheep bone oil.

牛乳蛋白纳米乳液体系不稳定,易发生絮凝和相分离等现象。为了改善纳米乳液的稳定性,需要对牛乳蛋白进行改性处理。以牛乳蛋白中的酪蛋白和乳清蛋白为研究对象,介绍了化学改性方法及效果,以及目前牛乳蛋白纳米乳液在食品中的应用情况。牛乳蛋白的化学改性方法主要有酸碱化、酰化、脱酰胺、磷酸化、糖基化等,其中糖基化是操作简便、效果显著的常用的牛乳蛋白改性方法。以改性的牛乳蛋白制备得到的纳米乳液,其冻融稳定性、乳化性以及抗氧化性得到改善。牛乳蛋白纳米乳液可以包封活性物质,实现靶向递送,在食品工业中应用广泛,也可以作为药物的稳定剂在制药行业应用。 The milk protein nanoemulsion system is unstable and prone to flocculation and phase separation. In order to improve the stability of the nanoemulsion, it is necessary to modify the milk protein. Taking casein and whey protein in milk protein as the research objects, the chemical modification methods and effects were introduced, as well as the current application of milk protein nanoemulsion in food. The chemical modification methods of milk protein mainly include acid-base, acylation, deamidation, phosphorylation, glycosylation, etc. Among them, glycosylation is a commonly used modification method of milk protein with the merits of simple operation and remarkable effect. The nanoemulsion prepared from the modified milk protein has improved freeze-thaw stability, emulsifying properties and antioxidant properties. Milk protein nanoemulsion can encapsulate active substances and achieve targeted delivery. The milk protein nanoemulsion system is widely used in the food industry and can also be used as a stabilizer for drugs in the pharmaceutical industry.

The proposal of the technical specifications for quality and safety production of oil-tea camellia seed oil can realize the standardization of products, and realize the upgrading and transformation of the industry by timely updating the technology and applying it to the standardization construction.

10. 高扬程多起伏输水管道停泵水锤防护优化.

In order to optimize the parameters of pump-stopping water hammer protection equipments for high-lift and multi-fluctuation water pipelines with considering reliability and economy, BP neural network was built to simulate the hydraulic transition process. The parameters of air valve and one-way surge tank were taken as decision variables, and the Pareto optimal solution set was obtained by cooperating with multi-objective genetic algorithm (NSGA-II). Entropy weight-TOPSIS method was used to determine the best decision. The method was applied to an engineering example. The results show that compared with the initial scheme, the optimized scheme provides similar positive pressure protection effect with the volume of the one-way surge tank less than 92.06%, and the minimum negative pressure along the pipeline is increased by 49.28%. Compared with no protective measures, while meeting the pressure resistance capacity of the pipeline, the maximum difference between the maximum and minimum pressure and yield capacity along the pipeline is decreased by 84.03% and 65% respectively. The method can realize the effective connection between the water hammer protection calculation and the intelligent optimization algorithm when the pump is stopped, and determine the reliable and economical parameters of the water hammer protection equipment when the pump is stopped [ABSTRACT FROM AUTHOR]

In order to efficiently and correctly identify abnormal behaviors in Linux logs, this paper proposes a Linux log anomaly detection method based on the improved isolated forest algorithm. The method introduces an attention mechanism on the basis of the isolated forest algorithm, which can dynamically adjust the attention features and sample points when processing log data, and dynamically adjust the degree of attention according to the degree of abnormality of the samples. Experimental results show that the method achieves high efficiency in the Linux log anomaly detection task compared with traditional methods, and can effectively discover potential security threats and abnormal behaviors. The occurrence of osteoporosis is due to the disruption of the balance between bone formation mediated by osteoblasts and bone resorption mediated by osteoclasts, resulting in abnormal bone homeostasis. MicroRNAs, as epigenetic regulatory factors, play an important role in bone metabolism by regulating the expression of signaling molecules related to osteoblasts and osteoclasts. This article reviews the latest research progress on the role of microRNAs in regulating and maintaining cell differentiation, proliferation, autophagy, and expression of related signaling molecules and pathways in bone microenvironment in osteoporosis, and summarizes the role of microRNAs in bone cell differentiation, highlighting the potential of microRNAs as therapeutic targets in metabolic bone diseases. Objective: To construct a new thrombus risk assessment model and evaluate its predictive ability for venous thromboembolism (VTE) in the patients with malignant tumors, and to provide the basis for the early predition of the malignant tumor patients with high risk for VTE. Methods: A total of 128 untreated malignant tumor patients were included, of which 40 were diagnosed with VTE within 2 months of malignant tumor diagnosis and categorized as VTE group. A total of 88 patients who did not develop VTE were categorized as non-VTE group. The clinical risk factors and laboratory indicators of the patients in two groups were compared and analyzed; the types of thrombotic events of the patients were analyzed; the diagnostic values of thrombin-antithrombin-complex (TAT), α2-plasmin inhibitor-plasmin complex (PIC), D-dimer(D-dimer), and fibrin degradation products(FDP) in malignant tumors complicated by VTE were assessed using receiver operating characteristic (ROC) curve analysis; Multivariate Logistic regression analysis was used to analyze the correlations of the clinical risk factors and biomarkers with the malignant tumors complicated with VTE. A new thrombus risk assessment model was constructed, consisting of TAT≥0. 70 μg·L-1, poor differentiation, and cardiovascular risk factors. The predictive probability of the model for malignant tumors complicated by VTE was evaluated based on the significance, goodness of fit, calibration curve, and C value of the model. The clinical application value of the new thrombus risk assessment model, COMPASS-CAT risk score (CRS), and Khorana risk score (KRS) in assessing malignant tumor patients complicated by VTE was compared using the C value and decision curve analysis (DCA). Results: The plasma levels of TAT (P<0. 001), PIC (P<0. 001), D-dimer (P<0. 05), and FDP (P<0. 01) of the patients in VTE group were higher than those in nonVTE group. Compared with the patients without cardiovascular risk factors, poor differentiation, and lymphatic metastasis, the malignant tumor patients with cardiovascular risk factors (P<0. 001), poor differentiation (P<0. 001), and lymphatic metastasis (P<0. 05) were more likely to develop VTE. Most VTE events (65%) were isolated deep vein thromboembolism(DVT). The ROC curve analysis showed that the area under the curve (AUC), sensitivity, and specificity of TAT and PIC were higher than those of D-dimer and FDP. TAT≥0. 70 μg·L-1 (P<0. 05), poor differentiation (P<0. 01), and cardiovascular risk factors(P<0. 01) were the independent risk factors for VTE in the malignant tumor patients. A new thrombus risk assessment model consisting of TAT≥0. 70 μg·L-1, poor differentiation, and cardiovascular risk factors was constructed. The new risk assessment model had a high goodness of fit (P=0. 805) and good predictive ability during internal validation (χ² =75. 266, P<0. 001). The ROC curve analysis results showed that the C values for the new thrombus risk prediction model, CRS, and KRS were 0. 908, 0. 676, and 0. 541, respectively. The DCA curve analysis results showed that the new thrombus risk assessment model had a higher net benefit rate compared with CRS and KRS. Conclusion: TAT and PIC have greater diagnostic efficiency than D-dimer in the early prediction of the malignant tumor patients with high-risk VTE. For the patients included in this study, the new thrombus risk assessment model, constructed from TAT≥0. 70 μg·L-1, poor differentiation, and cardiovascular risk factors, has superior diagnostic efficiency and clinical predictive value compared with CRS and KRS. Manure return to field has caused serious pollution of antibiotics and antibiotic resistance genes (ARGs) in soil-plant system, which has potential environmental risks. This paper summarized the distribution characteristics of antibiotics and ARGs in manure, soil, and plants and their influencing factors in soil-plant systems. The results showed that microorganisms prolonged exposure to low inhibitory concentrations of antibiotics and heavy metals was likely to induce the generation of ARGs, and there were significant geographical and species differences in antibiotic concentration and ARGs abundance in manure and improved soil. Generally, the residues of antibiotics and ARGs in manure and corresponding soil of pigs and chickens were higher than those of cattle. Moreover, antibiotics and antibioticresistant genes were more likely to accumulate in leafy and nightshade vegetables. Manure types, plant species, soil physicochemical properties, heavy metals, and microbial communities affected the distribution of antibiotics and ARGs in the soil-plant system. The migration of antibiotics from manure to soil and plants was greatly affected by the characteristics of antibiotics, and the concentration of antibiotics decreased continuously during transmedia migration. Bacterial communities significantly affect the migration of ARGs, diluted 100 to 1 000 times as they moved from soil to plant. For example, changes in soil microbial community structure will affect the species and abundance of ARGs in different cultivated crops. Research on the concentration control standards of antibiotics and ARGs in organic fertilizer and their migration mechanism needs to be intensified. This review aims to reveal the occurrence characteristics of ARGs in the soil-plant system and provide a basis for a scientific and effective response to the transmedia migration of microbial resistance. Ecological spatial functions support the high-quality development and construction of urban agglomerations, and needs to be strategically adjusted as its multi-functional needs change. This paper uses the kilometer grid as the evaluation unit and selects six functional indicators of climate regulation, water conservation, habitat maintenance, landscape conservation, soil conservation, and leisure recreation. And analyzes the evolution characteristics of the multi-functional ecological spatial in the Wuhan metropolitan area. Using hotspots analysis and random forest model methods, it explored the hotspot evolution and driving mechanism of the multifunctional ecological spatial. The results show that: (1) From 2000 to 2020, there were obvious differences in the evolution characteristics of ecological space in Wuhan urban circle. The functions of habitat maintenance, landscape conservation and soil conservation generally showed a weakening trend, and the functions of climate regulation, water conservation and leisure and recreation showed a general performance In order to enhance the trend, the leisure and recreation function has the largest increase. (2) During the study period, the spatial distribution patterns of the six types of ecological spatial functions have all changed significantly. The hotspots of water conservation and recreational functions showed an increasing trend as a whole, the hotspots of climate regulation and habitat maintenance showed a trend of first rising and then decreasing, and the hotspots of landscape conservation and soil conservation were basically stable. (3) Social-ecological factors have an important impact on the functioning of ecological space. On the whole, land use pattern, vegetation coverage and topographic factors have the greatest impact on the various functions of ecological space, but with the passage of time, the influence of socio-economic factors on the multi-functionality of ecological space has gradually increased. 关于浸出车间废水的排放要求,现行相关规范没有做出明确规定,在具体实施中,各浸出工厂根据自身实际制定企业执行标准,行业内存在排放措施差异较大的问题。对浸出车间工艺废水及冲洗废水、浸出禁区雨水、浸出车间事故液体的排放现状进行了阐述,通过类比相关规范标准,结合行业实际以及多年设计实践,针对排放措施提出了相关建议,同时对《浸出制油厂设计防火规范》的制定提出了建议。Regarding the discharge requirements of wastewater from leaching workshop, the current relevant codes do not make clear provisions, and in the concrete implementation, each leaching plant sets enterprise implementation standards according to its own reality, and there is a problem of large differences in discharge measures in the industry. The current situation of the discharge of process wastewater and rinsing wastewater from leaching workshop, rainwater from the leaching exclusion zone and accidental liquids from the leaching workshop was explained. By analogy with the relevant codes and standards, combined with industry practice and years of design practice, suggestions were proposed for the discharge measures, and suggestions were also proposed for the formulation of Code for Fire Protection Design of Oil Leaching System.

为了提高油脂原料仓液压滑模施工精度,以花生、大豆优质粮油生产项目为背景对筒仓液压滑模施工精度影响因素进行识别,运用ISM-MICMAC理论的方法建立影响因素多层递接模型,并以层次分析法(AHP)计算各影响因素的权重比。结果表明:操作平台没有足够的强度和适当的刚度、支承杆弯曲失稳及滑模装置组装不规范是造成滑模施工过程中精度控制不佳的最直观表现形式;液压千斤顶顶升不同步和外界温度过高或过低是影响施工精度的最重要因素。该分析方法明确了影响因素之间的作用关系,找出了影响施工精度的最重要因素,为原料筒仓壁施工的顺利完成提供了一定的参考。 In order to improve the construction accuracy of hydraulic sliding form for oil raw material silos, the factors affecting the construction accuracy of hydraulic slip form for silo were identified based on the peanut and soybean high-quality grain and oil production project. Using the method of ISM-MICMAC theory, the multi-layer recursive model of influencing factors was established, and the weight ratio of each influencing factor was calculated by analytic hierarchy process (AHP). The results showed that the lack of sufficient strength and proper stiffness of the operating platform, the bending instability of the support rod and the irregular assembly of the sliding form device were the most intuitive manifestations of poor precision control during the sliding form construction process. The asynchronous lifting of hydraulic jacks and the high or low external temperature were the most important factors affecting the construction accuracy. The method clarifies the relationship between the influencing factors, finds out the most important factors affecting the construction accuracy, and provides a certain reference for the smooth completion of the construction of the raw material silo wall.
