本研究の目的は「認知症高齢者の気になる行動」を中心に、生活者としての認知症高齢者と家族の抱える問題について分析し、認知症高齢者と介護者の生活支援の方法を検討することである。インタビューから得られたデータを抜粋し、KJ法により分類した結果、認知症高齢者の気になる行動は「もの忘れ」「怒りっぽい」「不安になる」「徘徊・うろうろする」「痛みがある・病気・身体の不調」「状況がわからない」「排泄の問題」「問題行動」といった8つのカテゴリーに分類された。生活上の最も困難な事象は認知症高齢者の問題行動であると考えられる。「徘徊・うろうろする」「認知症高齢者の不安から生じるさまざまな気になる行動」などは、いわゆるノーマルなものではなく、それなりの生活問題として重要であるが、最も困難な事象は弄便（ろうべん）であると考えられた。これらの問題行動は認知症という疾患が原因となるものであり、疾患を理解することで介護者の精神的な負担は軽減することが先行研究や本研究からも推察される。介護者は認知症高齢者の奇異な行動にも意味があることの理解をすることが、介護者の精神的な介護負担軽減に有用であると考えられた。これらの結果より、介護者への認知症高齢者の疾患の理解を促す支援が重要であることが示唆された。

わが国における認知症高齢者の増加，認知症施策推進総合戦略（新オレンジプラン）の策定などに伴い，これまで以上に認知症に対する関心が高まっている．しかし，認知症という言葉は身近になっているにもかかわらず，社会における認知症スティグマが軽減されているとは言い難い現状がある．このような背景から，認知症スティグマに着目した研究が増加しつつある．一方，認知症高齢者と高齢者ケア専門職，双方の認知症スティグマに着目した研究は少ない．そこで，本研究では文献研究の手法を用いて先行研究を踏まえ，本研究における認知症スティグマを定義するための理路を提示した．また，認知症のアセスメントとケアの現状と課題についても言及し，エビデンスとナラティヴに関する先行研究からの知見を援用した．以上の結果を統合し，実践への提言として，オノマトペ（擬音語・擬態語）を認知症ケアとアセスメントに使用することの可能性をまとめた．, Interest in dementia has been increasing recently due to the growing number of elderlypeople with dementia in Japan, and the formulation of the Japanese national dementia strategy（New Orange Plan）. Despite the situation, the stigma around dementia has not been reducedin society. While such background has increased the number of studies focusing on the stigmaaround dementia, there have only been a few studies focusing on the stigma of dementia fromthe perspectives of both the elderly with dementia and elderly care professionals. The aimof this study is to provide a rationale for defining the stigma of dementia based on review.Additionally, I mention the situation and challenges for dementia care and its assessment,and employ findings from prior studies on evidence and narrative. I integrate the results ofthe study and compiled the possibilities to use onomatopoeia （echoic and mimetic words） fordementia care and its assessment as a recommendation for dementia care practice.

ＤＳＭ−5では，認知機能をこれまでの5領域から6領域とし，新たに社会的認知が加わった．また，認知症施策推進総合戦略（新オレンジプラン）の策定に伴い，これまで以上に社会的認知が注目されるようになった．このような背景から，認知症高齢者の社会的認知を，科学的に検討する研究が増加しつつある．一方，高齢者ケア専門職の社会的認知を検討した研究は少ない．本研究では．認知症高齢者と高齢者ケア専門職，双方の社会的認知に着目し，認知症ケアの方法論として有益な知見を提示することを目的とした．本研究の目的を遂行するために，文献研究の手法を用いて，先行研究を踏まえ，本研究における社会的認知を定義するための理路を提示した・認知症ケアの方法論の現状と課題についても言及し，エビデンスとナラティヴに関する先行研究からの知見を援用した．以上の結果を統合し，実践への提言として，社会的認知をＳＯＡＰ形式にて，評価・記録することの可能性をまとめた．, The previous edition which included five domains of cognitive function has been updated to the newly revised edition of the Statistical Manual of Mental Disorders (DSM-5) which defines six domains of cognitive function including social cognition. Along with formulation of the Comprehensive Strategy to Accelerate Dementia Measures by the Japanese government (also called “New Orange Plan”), social cognition has drawn more attention. Based on such background, an increasing number of studies on social cognition in elderly people with dementia have been conducted, but only a few studies on social cognition in elderly care professionals have been performed. The present study, based on a literature review, focuses on social cognition in both the elderly with dementia and elderly care professionals, and is aimed at offering findings that are useful for providing dementia care. In order to achieve the objective of this study, a discussion is presented in the following order: a method of defining social cognition based on previous studies is shown; evidence and narrative are cited from previous studies; and recommendations for dementia care practice are provided from these integrated results.

本稿では、現代イタリアにおける高齢者施設での死について先行研究を中心に論じる。英米を中心とした高齢者施設における死にゆく過程についての研究では、予後診断が難しい認知症高齢者の場合、死にゆく者の役割を担うことができず、自らの死を主宰する権利をはく奪された高齢者の現状が指摘されている。一方、イタリアの高齢者施設においてSDM（共同意思決定）やAD（事前指示書）の欠如が指摘されているものの、その詳しい実態は分かっていない。ただ、高齢者施設に入ること自体がもつスティグマは事実としてあり、その意味では、施設死は「悪しき死」として否定されている。その悪しき死を避けるために、 在宅ケアを実現させようと低賃金で動員されるケア労働者は、家族愛を支えるための犠牲 者なのかもしれない。 The aim of this article is to describe some of the characteristic aspects of death in nursing homes in Italy by reviewing previous research. Some studies on the process of dying in nursing homes in Britain and the United States have shown that recognition of dying is medically and socially difficult. Consequently, the elderly (with dementia) cannot take their roles as dying persons, and have no right to decide how to die. On the other hand, the lack of documentation regarding shared decision-making has been pointed out in several nursing homes in Italy as well, but its details are unclear. Moreover, death in nursing homes is in fact stigmatized and denied as a bad death. To avoid such bad death, family caregivers hire migrant women (badanti) to care for the elderly at home until their death. Many burned-out migrant women, most of whom work with low wages may be victims of the ideal family image among Italians.

本稿は、Hochschildの感情労働において重要な概念である3つの構成特徴を整理し、その本質を論じたものである。先行研究に基づき、3つの構成特徴について理論的考察を行うとともに、感情労働における労働者、管理者、顧客の3極関係について、感情労働職として位置づけられてきた看護者、介護者、保育者を対象として言及することを目的とした。その結果、三職種における共通点として、上司が管理者であることが認められ、今後の展望として、感情労働者側の視点だけに限らず、管理者の意識を含めた多角的な側面から感情労働研究の重要性が示唆された。, The present paper clarifies three structural features which are key concept of Hochschild’s emotional labor; and, discuss the nature of it. Based on past researches, the three structural features are considered theoretically. It aims to mention nurse, care worker and childcare worker who have been positioned as emotional labor focusing on trilateral relationships of administrators, customers and workers in emotional labor. As a result, there are common points in the three different jobs that boss have been found to be an administrator. As the same time, the significance of emotional labor research from diverse aspects, including the consciousness of administrator has been suggested as future prospects. It is not limited to the point of view from the emotional labor’s side.

目的：回復期リハビリテーション病棟から自宅に退院する脳血管障害患者と介護者の主観的 QOL の特徴を明らかにし, 在宅に移行後の生活の再構築を促進するための看護介入の検討を行う. 方法：退院時に患者と介護者に半構造化面接により SEIQoL-DW index 値を測定し, Wilcoxn 順位検定, Spearman順位相関係数の算出, 質的分析を行った. 結果：対象者は16組であり, 患者群は男性11名, 女性５名, 平均年齢65.2±11.6歳, 平均入院期間76.5±27.7日, 退院時の平均 FIM 得点は105.4±30.1点であった. 介護者群は男性２名, 女性14名, 平均年齢63.8歳±12.8歳, 配偶者は11名であった. 主観的 QOL の評価である SEIQoL-DW index 値は, 患者群が63.2±20.8％, 介護者群は51.0±24.3％で, 有意な差は認められなかったが (p＝0.138), 相関を認めた(r＝0.521 p＝0.039). また, 両者の主観的 QOLに共通した事柄は【家族】【健康】で, 介護者のみが【介護者】と自分自身を挙げた. 考察：自宅に退院する時点での脳血管障害患者と介護者の主観的 QOL には差が無く, 両者が最も重要視していた生活の質の内容は【家族】であった. このため, 在宅に移行後の生活の再構築を促進するための看護介入として, 家族全体の生活を具体的に考えること, および介護者が自分自身の生活や健康を考えるための支援が必要と考える., Objective : To elucidate the impact of nursing intervention on the rehabilitation of the lifestyle of cerebrovascular disease patients who are discharged from recovery phase rehabilitation wards to their own home and their caregivers, and to clarify some characteristics of the patients' subjective QOL. Methods : We measured the Schedule for the Evaluation of Individual Quality of Life-Direct Weighting (SEIQoL-DW) index score through semi-structured interviews with the patients and their caregivers at the time of being discharged to home. We performed the Wilcoxon signed-rank test and calculated Spearman's rank-correlation coefficient and performed a qualitative analysis. Results : There were 16 patient/caregiver pairs. The patient group comprised 11 males and five females with a mean age of 65.2±11.6 years. Their mean hospital stay was 76.5±27.7 days. The mean Functional Independence Measure (FIM) score was 105.4±30.1 points. The caregiver group comprised two males and 14 females, including 11 spouses, with a mean age of 63.8±12.8 years. The index score, which is the evaluation of the subjective QOL, was 63.2±20.8％ for the patient group and 51.0±24.3％ for the caregiver group. No significant difference was found between them (p＝0.138), however, a moderate correlation was identified (r＝0.521 p＝0.039). According to the findings of the qualitative analysis, the common factors in the subjective QOL of both the patient group and the caregiver group were [family] and [health]. Discussion : The patients and their caregivers did not demonstrate any significant differences in the subjective QOL at the time of discharge from recovery phase rehabilitation wards. Both groups regarded the family as most important factor for subjective QOL. Increased support to help such patients better understand their relationship to family, and instruction to help improve the life and health of such caregivers is necessary.

回復期リハビリテーション病棟から在宅移行後１年以内のブローカ失語症者の介護者36名を対象として, 看護師による退院時の言葉に障害のある人への対応の指導の有無と, 在宅移行後の言葉の障害への対応の実際を自記式調査票で調査した. 退院時に言葉の障害に対する指導を受けた11名は, 指導を受けていない25名に比べ ｢ご本人が話す時は,言葉以外に身振り手振りや絵, 文字を使用してもらう｣ (p＝0.002 Wilcoxon 順位和検定) など失語症者に特有の対応方法を実施していた. このことから, 看護師が退院指導で失語タイプに適した代替手段を提供することで介護者は退院後の在宅生活でも適切に対応する可能性が示唆された., To assess the differences in the instructions provided at the time of discharge by nurses working in recovery phase rehabilitation wards, we distributed a self-administered questionnaire to 36 caregivers caring for patients with Broca's aphasia who were within a year after transitioning to home care. In comparison to 25 people who did not receive any instruction for speech disorders at the time of discharge, significantly high values in the peculiar countermeasure were found for 11 respondents who received instructions including : 〝When the patient speaks, encourage the patient to use body and hand gestures, pictures and characters in addition to words”(p＝0.002, Wilcoxon signed-rank test). These results suggested that the family care giver can improve both the patient outcome and their own efficacy by being provided with sufficient instructions by nurses.

国内外におけるパーキンソン病患者の介護者の負担に焦点を当てた文献を整理し，今後，介護者を支援してゆく上での課題を明らかにしてゆくことを目的とした．国内の文献は医中誌webを用いて検索した．海外の文献はWeb ofKnowledgeを用いて検索した．国内の文献は目的に該当した５件を本研究に活用した．海外の英文文献は目的に該当した４件を本研究に活用した．国内外の文献から，パーキンソン病患者の病気の進行度に応じて介護者が様々な体験を経験しており，それが介護者の精神状態と関連していることや，介護者の特性が精神的健康と関連している可能性が示された．今後の課題としては，患者の病気の進行に応じた介護者支援のあり方，介護者の特性に応じた介護者支援のあり方，患者の病気の回復を促すための介護者支援のあり方に焦点を当てた研究を進めることが挙げられた．, This study aims to accumulate international and domestic articles on the discussion of mental burdens experienced while caring for patients with Parkinson’s disease, and through the studies obtained, ascertain the direction for future research in this area. Domestic articles were retrieved through Igaku-chuuou-zasshi web,while foreign articles were retrieved through theWeb of Knowledge. Five domestic articles and nine English articles from foreign countries that were directly relevant to this study were used. Caregivers’experiences vary according to the disease progression oftheir patients. Additionally,their personalmental health,that is associated with their characteristics, plays a role in affecting their experience as a caregiver. Future research direction in this area includes encouraging more studies to ascertain caregivers’needs with relation to (1)the care given in accordance to disease progression, (2) differentiated support based on caregivers’ personal characteristics, and (3) updated studies on cure for Parkinson’s disease

Because of the attitude toward caregiving, any change in the lifestyle due to caregiving, mental conflict triggered by the start of caregiving, effort for administering care appropriately, mastering the skills for caregiving, and learning how to handle the burden and stress of caregiving to solve associated problems were extracted from the interviews. In addition, a new view of caregiving was gained through the experience, along with an increasing willingness to provide care based on affection for the care-receiver and the ties of marriage. It was observed that the caregivers changed their attitudes about caregiving as their role or the meaning of their lives, which strengthened the ties again.

本研究の目的は、①要介護高齢者の介護者が抱えている抑うつ感および介護負担感を明らかにすること、②過去に介護を行っていた介護者と現在介護を行っている介護者の抑うつ感および介護負担感を比較すること、③介護者の抑うつ感および介護負担感に関連のある諸要因を明らかにすること、④介護者の介護に対する意見や考えを聞き、介護の現状を明らかにすることであった。質問紙調査と面接調査を実施した。その対象は、X県の介護者家族会に参加をしている介護者男女約60名、面接調査の対象はX県の介護者家族会に参加をしている介護者女性5名であった。その結果、介護者が抱えている抑うつ感は『うつと認められない』対象者が多く、介護負担感では『中等度』の介護負担感を保持している対象者が多くいた。抑うつ感、介護負担感、認知症知識把握量は互いに関連し合っており、認知症の症状によっては認知症知識把握量が高いほど、抑うつ感、介護負担感が有意に高かった。精神面、身体面など、介護を始めてから発生した問題は数多く存在した。また、介護者が感じている負担感には、介護を介護として割り切ることや介護が終結しても介護に関係したことを行い続けること、介護を経験している者と関わることが関係しているのではないかと考えられた。

本研究の目的は、介護老人福祉施設で看取りケアに携わる介護者がどのような態度を取っているのか、その内容を明らかにすることである。方法は介護老人福祉施設で働く介護職員12名に対して印象に残った高齢者の看取りについてインタビューを行ない、それをGrounded Theory Approachを用いて分析した。その結果、介護者の看取りケアに対する態度は、看取りケアの捉え方と実践が明らかとなった。看取りケアの捉え方には、《高齢者の感情に気付く》、《死を自然に受け止められる》であり、看取りケアの実践には《孤独にさせないケア》、《ノルマ・マニュアルケア》、《ターミナル期だという判断》、《高齢者の意見の擁護》、《体制を整えて看取る》であった。高齢者に寄り添うことができるような看取りケアを実践するためには、高齢者の"できない・障害された部分"に注目するのではなく、病や加齢によって様々に障害された高齢者の中に"健康的な面を見出す"ことが必要であると考える。(著者抄録)

在宅人工呼吸療法へ移行した筋萎縮性側索硬化症(amyotrophic lateral sclerosis ; ALS) 療養者と主介護者の苦悩と対処法を明らかにすること, さらに苦悩の中で生きる主介護者にみられた健康要因をアントノフスキーのストレス対応能力概念SOC (Sense of Coherence：首尾一貫感覚) を構成する３つの要素(把握可能感, 処理可能感, 有意味感) の視点より検討し看護支援についての示唆を得ることを目的とした.ALS 療養者の苦悩には, 在宅で人工呼吸療法を続けることの不安, 目で表現していることをしっかり受け止めてくれない他者の存在と自分の存在価値への疑問, そして主介護者に対しては介護の負担をかけてしまう苦しみがあった. 従って, 在宅で人工呼吸療法を安心して続けられるための技術的支援, 療養者が家族や医療者から必要とされ存在価値を感じられるような看護支援, 介護者を支える看護支援が重要である.介護者の苦悩の中で最も強いものは延命選択への揺らぎであり, 療養者が生きる気力を失うことであった. 対処行動としては, 情報を識別しながら状況を理解し, 現状を受け止め諦めて他者の協力を得ていた. そして, この行動を支えているものは, 意味のある挑戦的な働きかけをし, 目標を持って生きるという人生の態度であり, ストレス対応能力概念SOC の中の有意味感に相当するものであった. 従って, 介護者に対する看護支援としては, SOC を構成する３つの要素のうち, 有意味感を特に重視して支援することが重要と考えられた., The purpose of this study is to clarify the distress in patients undergoing amyotrophic lateral sclerosis (ALS) treatment that shifted to mechanical ventilation at home as well as a method of solving this, and furthermore, to investigate the health factors observed in main caregivers living in distress from the point of view of the 3 elements (sense of comprehensibility ; sense of manageability ; sense of meaningfulness) making up the concept of the ability to cope with stress (SOC : Sense of Coherence) by AronAntonovsky to develop implications for nursing support. The distress in patients undergoing ALS treatment with an artificial respirator attached include : anxiety of continuing artificial respiration at home ; questions regarding the presence of others who do not seriously understand what is being expressed by the eyes and the meaning of the existence of the patient themselves ; and suffering from wanting the daughter, who is the main caregiver, to live life without feeling the pressure of care-giving, but not being able to achieve this. Accordingly, a variety of technical support is required in order for a patient to continue artificial respiration at home with ease. Moreover, nursing support that makes the patient feel that they are important to family members and medical workers and allows them to feel the meaning of their own existence, which may also support caregivers, is very important. Distress in caregivers includes : progression of the condition ; burden from care beyond expectations ; communication uncertainty and the dilemma of not being able to give priority to the children etc. However, the strongest distress is the struggle to choose to stay alive, which leads to caregivers sometimes losing their will to live. Regarding the actions of caregivers to cope, they understood the situation by distinguishing information and obtaining support from others by taking in the present condition. Their attitude towards life in working on meaningful challenges and living with an objective supports this action and is comparable to having a sense of meaning in terms of the concept of the ability to cope with stress, SOC. Accordingly, acting upon the 3 elements making up SOC is important as the nursing support towards caregivers ; however, it was believed that support must be given with serious consideration into the sense of meaningfulness.

【研究目的】認知症高齢者の情動・認知・行動に対して介護者がどのような働きかけを行っているのかについて、認知症高齢者の介護場面を観察することから明らかにする。【研究方法】介護職者6名に対して6名の研究者が、日中の6時間程度、参与観察を行い、その内容を細かくフィールドノートに記載し、疑問に思ったことなど対象者にインタビューを行った。参与観察とインタビューから得られた記録から、介護職員が認知症高齢者の情動や認知に働きかけて「その安定や変容を図っている」と記述された部分や「行動の習慣化(行動がスムーズに行えている)」と記述された部分を抜きだしてカテゴリー化した。【結果・考察】認知症高齢者の情動・認知・行動に対する介護者の働きかけとして、「行動を促す」働きかけと「安心感を与える」働きかけが明らかとなった。行動を促す働きかけは、、、、、であり、高齢者の情動・認知・行動に働きかけて日常生活行動を促すものであった。安心感を与える働きかけは、、、、であり、高齢者の情動・認知・行動に働きかけて高齢者の情緒安定や安心感を与えるものであった。(著者抄録)

認知症高齢者を在宅で介護している介護者の抱く介護の肯定的な認識と地域および特性の差異を明らかにすることを目的とした。調査は、認知症高齢者の介護者705人を対象者に郵送法を実施し、350人を分析対象者とした。分析方法は、介護充実感の因子分析、続柄、地域別による介護充実感の分散分析を行った。結果、介護充実感は、「達成感」「一体感」の2つの下位尺度で構成されていることが明らかになった。続柄の比較では、配偶者と嫁(p=0.019)で有意差が認められた。地域の比較では、有意差は認められなかった。本研究の結果から、認知症高齢者の介護者の抱く介護の肯定的な認識として、達成感、一体感の2要因の概念が明らかになった。臨床の場での、認知症高齢者の介護者の肯定的な認識の評価に、本研究で用いた介護充実感が活用できると考えられる。介護の肯定的な認識は、続柄による違いが認められ、続柄別による支援方法を検討する必要があるといえる。(著者抄録), The purpose of the present work was to identifyp ositivea ppraisal,r egional difference,a nd characteristics for caregiverso f elderly with dementia at home. 本研究の目的は、グループホーム等小規模多機能型居宅介護施設での看取りの実態を明らかにし、その実態からグループホーム等小規模多機能型居宅介護施設が目指す「豊かな看取り」とは何であるのかを提言することである。調査はグループホーム、小規模多機能型居宅介護施設(合計7施設)の看護職・介護職等14名を対象に、半構成的インタビュー調査を行い、質的帰納的に分析した。結果、を行う際に、が前提にあった。さらに、今回のグループホーム等小規模多機能型居宅介護施設でのインタビュー調査から、「豊かな看取りケア」とは、(1)高齢者の意思を尊重したいと思えること、(2)寄り添いたいと心から思い、時を一緒に過ごし、見守るゆとりをつくること、(3)出来る限りの苦痛を取り除きたいと思えること、(4)看取り期の生きている今を支えることの4点であった。

This paper culls the highlights of Filipino interviewees’ experiences, comments, and suggestions about their own participation in this new Economic Partnership Agreement (EPA) program which brings together the Filipino nurses, caregivers and the Japanese elderly within the context of Japanese hospitals and caregiving institutions. Although limited by the small sample size, the interview results can serve as initial assessment of the EPA program and can also be used as baseline information for more comprehensive future research and policy. The inclusion of the movement of health and care providers within the EPA is also worth analyzing, especially in terms of its implication for care which has gone beyond homes and households, for the care providers and recipients, for gender, as well as for the participating governments and their respective societies.

本研究は,手浴ベースンの開発とその評価を行ったものである.本手浴ベースンのデザインの特徴は,患者の手を湯に深く浸せると同時に,ベッド上でも安定しやすいことである.6病院25病棟(療養型14,急性期11)において,本手浴ベースンを用い手浴を実施した130名の介護・看護職全員にアンケートを行った.アンケートの結果,手浴ベースンの基本的な形状である高さ,横幅,奥行き,曲面の傾斜が調度よいと回答した人は約70%であった.使いやすいと回答した人は60%であった.その理由で最も多かったことは,曲面が患者の体にフィットし安定することであり,次いで患者の手を深く湯に浸せられることであった.使いにくいと回答した人は15%であった.その主な理由は側臥位が不安定な患者には合わないこと,拘縮の程度が重い場合は手を入れにくいことであった.本手浴ベースンは側臥位が可能で,拘縮手の程度が比較的経度な患者であれば使い勝手がよく有効であることが示唆された.(著者抄録)

グループホームでの認知症高齢者への終末期ケアの提供に際し、看護師に期待される役割を明らかにする目的で、N県内のグループホームを対象に半構成的な面接調査を行った。面接内容の逐語録からグループホームでの終末期ケアに関する看護師の役割についての語りを取り出し、内容の類似性に着目して分類整理を行った。結果、【共に居る】、【健康管理を行う】、【日常生活を支える】、【必要な医療を提供をする】、【医療と連携する】、【インフォームドコンセントを支える】、【相談に応じる】、【スタッフ教育を行う】の8カテゴリーが抽出され、更にこれらは、、の3側面に整理された。以上の結果から、看護師はグループホームでの終末期ケア提供にあたり、のみならず、と同時にといった役割が期待されていた。(著者抄録)

民生委員は最も地域に根ざした福祉の担い手である。この民生委員に対する高齢者虐待の意識調査をおこなった。アンケート調査の対象は、I市の民生委員定数195人で、回答者は173人であった。(回答率88.7%)。性別は男性83人、女性82人で男女の差はなかった。年齢層は、60歳代が最も多く、51.4%、70歳代は20.2%であり、40歳代5.8%と最も少なかった。民生委員の在任期間は、3年が51人(29.5%)で最も多く、次いで6年が37人(21.4%)であり、全体の50.9%を占めている。9年以上の在任期間を有する者は全体の27.9%であった。民生委員の職業は、特徴的な傾向はないが、主婦が最も多く63人(38.4%)であった。僧侶が6人(3.5%)が目立つ程度である。民生委員の高齢者虐待の体験では、10.4%の人が虐待を発見している。高齢者虐待は増加していると考える人が63.0%であった。民生委員の今後の活動では、虐待の研修や普及活動が必要であると答えた人が、それぞれ、68.2%、66.5%であり、民生委員として高齢者虐待防止の活動に対する積極性がうかがえた。虐待の通報先では、市町村や地域包括センターなど公的機関と答えた人が64.5%であった。虐待のイメージに関しては身体的暴力が83.4%の人が虐待と感じており、暴言は78.6%、無視、世話放棄は、約70%の人が虐待と認知していた。高齢者虐待の要因は介護負担、家族関係によるものが約80%であった。民生委員法の改正に伴う民生委員の「名士」的な役割から抜け出し、住民との対等な立場で地域の福祉に積極的に関わる民生委員の活動について言及している。今後の民生委員の積極的な関わりが期待できるアンケート結果であった。, 6, KJ00005164999

P(論文), 在宅介護を継続している家族介護者が介護継続意欲を持つ要因を明らかにし、家族介護者が介護継続意欲を持つことができる援助を考察することを目的とした。在宅介護を継続している家族介護者1名を対象に、半構成的面接を実施し、質的帰納的分析を行った。その結果、介護継続意欲を持つ要因として計14のカテゴリーが得られ、介護者自身に関連するもの、要介護者に関連するもの、サポートに関連するものに分類された。介護者が介護継続意欲を持てるような援助として、介護者が介護の価値を見出す、介護の成果を実感できる、家族の絆を再確認できる、気分転換を図れる、肯定的な評価等の関わりが示唆された。

Due to its rapidly aging society, Japan is facing critical issues related to care, including methods for end-of-life care for the elderly. The objective of the present study was to elucidate factors related to homecare.Questionnaires were administered to care recipients and caregivers of individuals using visiting nursing services within Prefecture A. Analysis was performed for a total of 338 care recipients and caregivers. Odds ratios for factors related to the desired location of care among caregivers were determined using multiple logistic regression analysis.The following factors were identified as factors related to desire for homecare among caregivers: 1) high care requirement level, 2) desire among care recipients for end-of-life care at home, 3) unfavorable impression of hospital care among caregivers, 4) favorable impression of homecare among caregivers, and 5) desire among the caregivers themselves for end-of-life care at home.These findings suggest that support for recognizing the preferences of care recipients and social support for enabling caregivers to comfortably provide care are necessary for future provision of homecare., わが国では急激な高齢化を迎え，高齢者の終末期ケアのあり方を含めた看取りの問題が重要な課題となっている．本研究では，自宅での看取りに関連する要因を明らかにすることを目的とした．A県内全域の訪問看護サービスを利用している療養者と介護者にアンケート調査を行い338組を分析対象とした．多重ロジスティック回帰分析を用い，介護者の自宅での看取り希望に関連する要因のオッズ比を求めた．介護者の自宅での看取り希望に関連する要因は，1）要介護度が重度であること，2）療養者が自宅での終末期を希望していること，3）介護者が抱く病院の看取りイメージが悪いこと,4）介護者が抱く自宅の看取りイメージが良いこと，5）介護者自身の終末期を迎えたい場所が自宅であることであった．今後，自宅での看取りを可能にするためには，療養者の意向を認識できる支援や，介護者が安心して看取りたいと思えるような社会的支援が重要であると示唆された．

介護者の感染対策の実施状況を明らかにするため,研究の同意が得られた介護者を対象に調査を実施した。調査には,対象者の基本属性,手洗い行動,感染対策への意識,インフルエンザワクチン接種状況についてのアンケート用紙を作成した。その結果,153 名の有効回答が得られた。結果として,1)おむつ交換後の手洗い実施率は高い(92.9%)。2)しかし,帰宅時の手洗いの実施率は低かった(67.3%)。3)介護開始後に手洗いの指導を受けていない介護者が多い(34.6%)。4)介護者,療養者共に6割以上がインフルエンザワクチンを接種していた。これらより,介護者の感染予防行動をより高めていく為に,基本的な感染管理について指導していくことが重要であると考えられた。

This research focuses on the problems of family care in Japan and discusses a social work practice model that can explain these problems. Applying the concept of symbolic interaction as the framework for analysis, I tried to develop an integrated theory that would explain the meaning of family care and that would guide social work interventions. The result of the study indicates that the meaning of family care differs from one caregiver to another, depending on the experience of each caregiver and on the interpretation of experience by each caregiver. In addition, I introduced a new concept of "burying the past" for the purpose of better understanding the problems. These new theories are expected to be useful for preventing and intervening on the problems of family care in Japan.

Purpose : To enlighten the nursing personnel on oral care to the dependent elderly, a pamphlet was developed in collaboration with dentists, nurses and dental hygienists. A questionnaire survey on the understanding of the nursing personnel and the pamphlet utilizing condition was carried out. Materials and methods : The pamphlet was written in plain terms and was mainly constituted of photos and illustrations, to underscore the importance and to explain how to practice the oral care of the dependent elderly. The pamphlets were mailed to the nursing personnel in charge of 46 nursing-care facilities in Niigata city, accompanied with a questionnaire that asked about the present oral care conditions of the residents, the pamphlet utilizing conditions and changes in the oral conditions after the pamphlet use. Results and discussion : Answers were obtained from 27 facilities (the collect rate was 58.7%). All nursing personnel answered that they could understand the contents and they used the pamphlet for the staff training in oral care practice methods. It was suggested that this pamphlet increased the staff recognition of oral care importance and helped in the practice of oral care in nursing-care facilities., 【目的】要介護高齢者への口腔ケアを定着させる目的で､歯科医師､看護師､歯科衛生士が連携して､介護者の口腔ケア意識を啓発するためのパンフレットを作成した｡このパンフレットに対する介護者の理解と活用状況をアンケートにて評価した｡【方法】パンフレットは要介護高齢者への口腔ケアの必要性と実践法を啓発する内容とし､平易な用語を用い､写真やイラストを中心にデザインした｡新潟市内介護施設(計46施設)の介護担当責任者にパンフレットを配布し､口腔ケアの現状､パンフレットの活用状況､活用後の変化に関するアンケートを郵送法で送付､回収した｡【結果および考察】回収率は58.7%(27施設)であった｡回収できた27施設すべてから内容が｢理解できた｣との回答が得られ､作成したパンフレットは介護者にとってわかりやすいものであった｡パンフレットはスタッフ教育やケア方法の取得などスタッフ間での活用が多かった｡パンフレット活用後には､スタッフの口腔ケアの認識向上､ケア方法の確立､ケアを新たに実践などの変化が見られた｡このように､今回のパンフレットは介護者の口腔ケアへの知識や意識を高めるとともに､介護施設で口腔ケアを実践するきっかけとなったと推察できた｡

ターミナル期にある家族を在宅で介護し、在宅で看取りをした介護者の思いと介護者側からみた訪問着護師(以下Nsとする)の支援を明らかにするために、3事例の主介護者(以下介護者とする)に半構成的面接を行い、グラウンデッド・セオリー・アプローチの継続的比較法を用い質的に分析した。介護者の思いには療養者との「関係性」が影響し、介護者とNs、療養者とNs間の関係性にも注目して分析した結果、『維持したい関係性の本質』をコアカテゴリーとした。介護者と療養者には『本質の前提となる今までの歴史』と『本質の前提となる現状の関係』があるが、『維持したい関係性の本質に影響したNsの支援』を非常に役立ったと感じていた。従ってこれらをふまえたアセスメントの視点をもつことが家族への有効な支援につながることが示された。また、介護者の『在宅での看取りのアウトカム』には4つの局面があり、影響する要因もカテゴリーとして明らかになった。

グループ回想法の効果と意義を明らかにすることを目的に,療養型病院の痴呆性高齢者6名および介護老人保健施設の痴呆性高齢者7名にそれぞれ実施したグループ回想法を録画したビデオを,痴呆性高齢者の家族3名および現場のグループ回想法のスタッフ11名に視聴してもらい,その後,面接を実施した.その結果,家族とケアスタッフが捉えたグループ回想法の効果と意義として,以下が明らかになった.1)痴呆高齢者に潜んでいた豊かな感情の表出や,心理的安定が図られたこと.2)回想の刺激材料によって,ほとばしり出てきた過去の記憶を,痴呆性高齢者が熱意をもって語っていたこと.3)家族自身が高齢者とともに生きた人生を振り返る意義.4)家族が高齢者との関係性を振り返り,つながりを見出す意義.5)ケアスタッフが痴呆性高齢者の能力を再発見した意義

重症心身障害児(者)の介護者の精神的な健康度と介護負担感や介護状況について明らかにすることを目的として,Y県の重症心身障害児(者)を守る会の協力を得て,全会員の中から在宅で生活する障害児(者)の介護者49人を対象とし,アンケート調査を実施した。有効回答数は33であった。介護者の平均年齢は64.8±8.7歳,そのうち31人が障害児(者)の母親であった。最も介助を必要とするのは入浴(全介助:16人)で,逆に食事は介助なしが17人であった。介助を要する場合,約半数は副介護者がおり,その多くは父親であった。副介護者の存在は,介護負担感を低めていた。介護負担感の高さ,介護不安があること,障害児(者)が身体障害者手帳と療育手帳の2種類を有することが精神的健康度を低めていた。

わが国の人口の高齢化は急速に進み,2000年では全人口の6人に1人であった高齢者(65歳以上)が,2025年には3人に1人になると予測されている。それに伴い,痴呆性高齢者の数も確実に増加し,在宅で痴呆性高齢者を介護する家族も増加することが考えられる。今回著者らは,介護者だけでなく介護を受けている痴呆性高齢者に対しても家庭訪問による聞き取り調査を行い現状を把握した。調査の結果,痴呆性高齢者の問題行動は徘徊が最も多く,そのため介護者は自分の時間が持てないことが一番の悩みであった。しかし訪問したうち多くの家庭では,それぞれ介護負担はあるが,介護サービスを利用して介護状況は安定していた。保健師は専門職として介入する役割が求められていることが明らかになった。, As the population of Japan increases, the population of aged people (more than 65 years of age) increases, from one in six in 2000 to probably one in three in 2025. 本研究の目的は、痴呆性老人を在宅で介護する家族が虐待に至ってしまう要因は何かについて、事例および文献を通して明らかにすることである。 まず、文献を通して、高齢者虐待の定義を明確にした。次に、実際に痴呆性老人を介護しながら、虐待には至っていない家族への面接を通して、介護の場面では、何らかの精神的・身体的負担が重なりあうと、虐待という行動に至ってしまわざるを得ない状況を生じてしまうことが分かった。医療・保健・福祉の専門家は連携して、これらの発生的要因を早期に断ち切り、介護家族への相談・保護・支援をする

本論は、病気により身体の自由を失い、病院でリハビリテーション(以下リハビリと略す)を行っていたが、効果が期待できず在宅療養となった患者(M氏)のケアを振り返り、高齢者の自己概念･自己尊重の低下について考えたものである。私は訪問看護婦としてM氏に関わり、在宅で受けることができる保健医療サービスを受け、恵まれた環境で療養生活を送っていると思っていた。しかしM氏の全身状態は安定していたが、精神的に不安定であった。私はそのようなM氏に対してどのように援助すべきかを見出せず大変苦悩した。そこで今回この事例のケアを振り返ることで、脅かされやすい高齢者の自己概念･自己尊重の低下という視点が欠けており、その視点が重要であったということに気づくことができたので報告する。
