Differences in the speech and language outcomes for family care givers who received discharge instructions for patients with Broca's aphasia

Abstract

回復期リハビリテーション病棟から在宅移行後1年以内のブローカ失語症者の介護者36名を対象として, 看護師による退院時の言葉に障害のある人への対応の指導の有無と, 在宅移行後の言葉の障害への対応の実際を自記式調査票で調査した. 退院時に言葉の障害に対する指導を受けた11名は, 指導を受けていない25名に比べ 「ご本人が話す時は,言葉以外に身振り手振りや絵, 文字を使用してもらう」 (p=0.002 Wilcoxon 順位和検定) など失語症者に特有の対応方法を実施していた. このことから, 看護師が退院指導で失語タイプに適した代替手段を提供することで介護者は退院後の在宅生活でも適切に対応する可能性が示唆された.<br />To assess the differences in the instructions provided at the time of discharge by nurses working in recovery phase rehabilitation wards, we distributed a self-administered questionnaire to 36 caregivers caring for patients with Broca's aphasia who were within a year after transitioning to home care. In comparison to 25 people who did not receive any instruction for speech disorders at the time of discharge, significantly high values in the peculiar countermeasure were found for 11 respondents who received instructions including : 〝When the patient speaks, encourage the patient to use body and hand gestures, pictures and characters in addition to words”(p=0.002, Wilcoxon signed-rank test). These results suggested that the family care giver can improve both the patient outcome and their own efficacy by being provided with sufficient instructions by nurses.
