Stereotype rebound occurs when people experience an increase in stereotype use following attempts to suppress them. Two studies were conducted to examine cultural variability in the rebound effect. We hypothesized that, due to their experience suppressing unwanted thoughts, people from collectivist cultures would be less likely to experience the rebound effect than would people from individualist cultures. In both studies, U.S. and Chinese participants wrote two stories about gay men that were coded for stereotype use. Participants in the suppression condition were instructed to not to use stereotypes in their first story. As predicted, in both studies, U.S. participants in the suppression condition showed an increase in stereotype use in the second story, but Chinese participants did not. Potential explanations for cultural differences in the stereotype rebound effect are discussed.

Background and Aims: Trichoderma species have potential in biocontrol of eutypa dieback; however, little is known about their ability to colonise grapevine wood. The colonisation of vines by Trichoderma harzianum and its effects on colonisation by Eutypa lata were studied. Methods and Results: T. harzianum colonised canes over a total distance (above and below the inoculation point) of 10 cm or more in 12 weeks after inoculation. Application of T. harzianum to canes prior to E. lata, either at the same inoculation point or at a different point, reduced recovery of the pathogen. In the field, T. harzianum grew 3 cm from the inoculation point in 4 months in all seven of the mature vines treated. The fungus persisted for 20 months in four of the seven vines at 6 cm from the inoculation point and reached the crown of one vine. Prior inoculation of mature vines with Trichodowels, containing T. harzianum, reduced recovery of E. lata 14 months after inoculation. Conclusions: T. harzianum colonised wood of Chardonnay, Shiraz and Nyora and reduced colonisation by E. lata. T. harzianum persisted in wood of mature vines of Nyora for up to 20 months. Significance of the Study: The results support the potential of T. harzianum in the biocontrol of eutypa dieback.

In a prospective study from 1972 to 1982, 38 patients with bleeding oesophageal varices and relatively good liver function were treated by semi-elective or emergent shunt 2-4 weeks after admission. There were no deaths within 3 months of surgery. This constituted 30% of the total 125 patients admitted with bleeding varices during that period. No referred case for treatment of portal hypertension was included in this study. The procedures performed were end-to-side portacaval shunt in 30 cases, lieno renal shunt in seven cases and interposition mesenterico-caval shunt in one case. All patients were admitted to a special unit with a two stage policy of management. Immediate endoscopic diagnosis and balloon tamponade in those with continued bleeding was followed by shunt in selected cases. Follow-up of the 38 patients showed a cumulative survival at 1 year of 89%, at 3 years of 75% and 5 years of 65%. In four cases (13%) a clinical diagnosis of portal systemic encephalopathy was made, all were controlled by medical management. In four of ten follow-up deaths, liver failure was the cause, in none of these cases was encephalopathy a problem. It is concluded that with a policy of early diagnosis and control of haemorrhage, 30% of a typical series of prospectively studied patients admitted with bleeding varices can be treated without mortality by definitive surgery. There was low incidence of encephalopathy and no cases of incapacitating mental confusion. Centralization of treatment and prospective study is essential for the implementation of such a policy of management.

Eight patients with haemorrhage from smooth muscle tumours of the upper gastro-intestinal tract were treated during a 10 year period from 1973 to 1982. Seven of the tumours were benign and one a malignant leiomyoblastoma. Seven tumours were sited in the stomach and there was one duodenal lesion. Endoscopy was performed in all eight cases and made the diagnosis definitively in four. In two cases the diagnosis was confirmed on barium meal and in the other two, diagnosis was eventually made at laparotomy. Two patients were shocked on admission and required emergency surgery. In one case a diameter greater than 10 cm suggested malignant potential and wide local resection was performed. In one other case with a tumour in the antrum, a distal partial gastrectomy was performed, and in the case with leiomyoblastoma a proximal gastrectomy was performed. One case was lost to follow-up. The mean follow-up in six cases free of disease with benign tumour was 2.6 years. The patient with a tumour of greater than 10 cm in diameter remains well at 18 months follow-up and the patient with a malignant leiomyoblastoma died 2 years after surgery.

Models have been developed to predict the probability that a person carries a detectable germline mutation in the BRCA1 or BRCA2 genes. Their relative performance in a clinical setting is unclear.To compare the performance characteristics of four BRCA1/BRCA2 gene mutation prediction models: LAMBDA, based on a checklist and scores developed from data on Ashkenazi Jewish (AJ) women; BRCAPRO, a Bayesian computer program; modified Couch tables based on regression analyses; and Myriad II tables collated by Myriad Genetics Laboratories.Family cancer history data were analyzed from 200 probands from the Mayo Clinic Familial Cancer Program, in a multispecialty tertiary care group practice. All probands had clinical testing for BRCA1 and BRCA2 mutations conducted in a single laboratory.For each model, performance was assessed by the area under the receiver operator characteristic curve (ROC) and by tests of accuracy and dispersion. Cases "missed" by one or more models (model predicted less than 10% probability of mutation when a mutation was actually found) were compared across models.All models gave similar areas under the ROC curve of 0.71 to 0.76. All models except LAMBDA substantially under-predicted the numbers of carriers. All models were too dispersed.In terms of ranking, all prediction models performed reasonably well with similar performance characteristics. Model predictions were widely discrepant for some families. Review of cancer family histories by an experienced clinician continues to be vital to ensure that critical elements are not missed and that the most appropriate risk prediction figures are provided.

Knowledge about eating disorders influences lay people's ability to recognize individuals with anorexia nervosa (AN) and bulimia nervosa (BN) and refer them to professional treatment. We assessed mental models (stored knowledge) of AN and BN in 106 college students. Results indicated that most students have general, but not specific, information about AN and BN's symptoms, consequences, causes, duration, and cures. They also believe that people with eating disorders tend to be young, White women. These findings suggest that lay recognition of eating disorders may be based primarily on observations of dysfunctional eating behaviors and therefore facilitated by additional knowledge.

HLA-G is an HLA class Ib gene that is highly expressed in human trophoblast cells. The single HLA-G mRNA is alternatively spliced to generate at least seven transcripts, three of which encode soluble isoforms. Many studies have shown that high levels of soluble antigens are associated with successful implantation and graft acceptance. To study expression, regulation and functions of two of the soluble isoforms, HLA-G5 and HLA-G6, we generated recombinant proteins in eukaryotic cells and developed monoclonal antibodies specific for each of the two proteins. In addition, we investigated the olive baboon Paan-AG gene as a potential functional correlate of HLA-G. Here, we present summaries of the studies that have been conducted in our laboratory using these tools and discuss the results within the context of the research on this topic that is ongoing in ours and other laboratories worldwide. Collectively, the data indicate that soluble HLA-G is a critical contributor to immune privilege in pregnancy and imply that this placenta-derived substance may impact other pathways leading to successful reproduction.

This article summarizes the proceedings of a symposium organized by Mark Stanton and Pamela Hunt and presented at the annual meeting of the International Society for Developmental Psychobiology. The purpose of the symposium was to review recent advances in neurobiological and developmental studies of fear and eyeblink conditioning with the hope of discovering how neural circuitry might inform the ontogenetic analyses of learning and memory, and vice versa. The presentations were: (1) Multiple Brain Regions Contribute to the Acquisition of Pavlovian Fear by Michael S. Fanselow; (2) Expression of Learned Fear: Appropriate to Age of Training or Age of Testing by Rick Richardson; (3) Trying to Understand the Cerebellum Well Enough to Build One by Michael D. Mauk; and (4) The Ontogeny of Eyeblink Conditioning: Neural Mechanisms by John H. Freeman. Taken together, these presentations converge on the conclusions that (1) seemingly simple forms of associative learning are governed by multiple "engrams" and by temporally dynamic interactions among these engrams and other circuit elements and (2) developmental changes in these interactions determine when and how learning emerges during ontogeny.

We report a significant reduction in the rate of meticillin-resistant Staphylococcus aureus (MRSA) transmission on a critical care unit when admission screening by culture was replaced with a same-day polymerase chain reaction (PCR) test. This was an observational cohort study, set in a 19-bed mixed medical and surgical adult critical care unit in southwest England. We studied 1305 patients admitted between April 2005 and February 2006. Standard MRSA culture methods were used to screen 612 patients between April 2005 and August 2005, and the IDI MRSA PCR test was used to screen 693 patients between September 2005 and February 2006. Standard infection control precautions were instituted when positive results were obtained by either method. Outcome measures included carriage rate, turnaround time for results and rate of subsequent MRSA transmission on the unit. The overall carriage rate on admission to the unit was 7.0%. Culture results were available in three working days, PCR results within one working day. The mean incidence of MRSA transmission was 13.89/1000 patient days during the culture phase and 4.9/1000 patient days during the PCR phase (relative risk reduction 0.65, 95% CI 0.28-1.07). PCR screening for MRSA on admission to critical care units is feasible in routine clinical practice, provides quicker results than culture-based screening and is associated with a significant reduction in subsequent MRSA transmission.

The electronic health record (EHR) may be an effective tool to help clinicians address tobacco use more consistently. To evaluate the impact of EHR-generated practice feedback on rates of referral to a state-level tobacco quitline, we conducted a cluster randomized clinical trial (feedback versus no feedback) within 19 primary care clinics in Oregon. Intervention clinics received provider-specific monthly feedback reports generated from EHR data. The reports rated provider performance in asking, advising, assessing, and assisting with tobacco cessation compared with a clinic average and an achievable benchmark of care. Experience-produced deficits in trace conditioning and context conditioning have been useful tools for examining the role of the hippocampus in learning. It has also been suggested that learning in these tasks is especially vulnerable to neurotoxic effects of alcohol during key developmental periods such as adolescence. In five experiments we systematically examined the presence and source of age-dependent vulnerability to the memory-disrupting effects of acute ethanol in trace conditioning and contextual fear conditioning. In Experiment 1a pre-training ethanol disrupted trace conditioning more strongly in adolescent (postnatal day, PD30-35) than adult rats (PD65-75). In Experiment 1b when pre-training ethanol was accompanied by pre-test ethanol no deficit in trace conditioning was observed in adolescents, suggesting that state-dependent retrieval failure mediated ethanol's disruption of trace conditioning at this age. Experiments 2a and 2b examined the effect of ethanol pretreatment on context conditioning. Here, adult but not adolescent rats were impaired in conditioned freezing to context cues. Experiment 2c explored state-dependency of this effect. Pre-training ethanol continued to disrupt context conditioning in adults even when ethanol was also administered prior to test. Collectively these findings reveal clear age-dependent and task-dependent vulnerabilities in ethanol's disruptive effects on hippocampus-dependent memory. Adolescents were more disrupted by ethanol in trace conditioning than adults, and adults were more disrupted by ethanol in context conditioning than adolescents. We suggest that adolescents may be more susceptible to changes in internal state (state-dependent retrieval failure) than adults and that ethanol disrupted performance in trace and context conditioning through different mechanisms. Relevance of these findings to theories of hippocampus function is discussed.

We compare the stellar motion around a spiral arm created in two different scenarios, transient/co-rotating spiral arms and density-wave-like spiral arms. We generate Gaia mock data from snapshots of the simulations following these two scenarios using our stellar population code, SNAPDRAGONS, which takes into account dust extinction and the expected Gaia errors. We compare the observed rotation velocity around a spiral arm similar in position to the Perseus arm, and find that there is a clear difference in the velocity features around the spiral arm between the co-rotating spiral arm and the density-wave-like spiral arm. We investigate the kinematic signatures induced by spiral and bar structure in a set of simulations of Milky Way-sized spiral disc galaxies. The set includes test particle simulations that follow a quasi-stationary density wave-like scenario with rigidly rotating spiral arms, and $N$-body simulations that host a bar and transient, co-rotating spiral arms. From a location similar to that of the Sun, we calculate the radial, tangential and line-of-sight peculiar velocity fields of a patch of the disc and quantify the fluctuations by computing the power spectrum from a two-dimensional Fourier transform. We find that the peculiar velocity power spectrum of the simulation with a bar and transient, co-rotating spiral arms fits very well to that of APOGEE red clump star data, while the quasi-stationary density wave spiral model without a bar does not. We determine that the power spectrum is sensitive to the number of spiral arms, spiral arm pitch angle and position with respect to the spiral arm. However, it is necessary to go beyond the line of sight velocity field in order to distinguish fully between the various spiral models with this method. We compute the power spectrum for different regions of the spiral discs, and discuss the application of this analysis technique to external galaxies., Comment: 14 pages, 11 figures. We present a new theoretical population synthesis model (the Galaxy Model) to examine and deal with large amounts of data from surveys of the Milky Way and to decipher the present and past structure and history of our own Galaxy. We assume the Galaxy to consist of a superposition of many composite stellar populations belonging to the thin and thick disks, the stellar halo and the bulge, and to be surrounded by a single dark matter halo component. A global model for the Milky Way's gravitational potential is built up self-consistently with the density profiles from the Poisson equation. In turn, these density profiles are used to generate synthetic probability distribution functions (PDFs) for the distribution of stars in colour-magnitude diagrams (CMDs). Finally, the gravitational potential is used to constrain the stellar kinematics by means of the moment method on a (perturbed)-distribution function. Spiral arms perturb the axisymmetric disk distribution functions in the linear response framework of density-wave theory where we present an analytical formula of the so-called `reduction factor' using Hypergeometric functions. Finally, we consider an analytical non-axisymmetric model of extinction and an algorithm based on the concept of probability distribution function to handle colour magnitude diagrams with a large number of stars. A genetic algorithm is presented to investigate both the photometric and kinematic parameter space. We have developed our original made-to-measure (M2M) algorithm, PRIMAL, with the aim of modelling the Galactic disc from upcoming Gaia data. From a Milky Way like N-body disc galaxy simulation, we have created mock Gaia data using M0III stars as tracers, taking into account extinction and the expected Gaia errors. In PRIMAL, observables calculated from the N-body model are compared with the target stars, at the position of the target stars. Using PRIMAL, the masses of the N-body model particles are changed to reproduce the target mock data, and the gravitational potential is automatically adjusted by the changing mass of the model particles. We have also adopted a new resampling scheme for the model particles to keep the mass resolution of the N-body model relatively constant. We have applied PRIMAL to this mock Gaia data and we show that PRIMAL can recover the structure and kinematics of a Milky Way like barred spiral disc, along with the apparent bar structure and pattern speed of the bar despite the galactic extinction and the observational errors., Comment: 4 pages, 1 figure, to appear in the proceedings of "The Milky Way Unravelled by Gaia: GREAT Science from the Gaia Data Releases", Barcelona, 1-5 December 2014, eds. The principal role of the immune system is to defend the integrity of the host by preventing infectious disease, tumor development, and contamination of the host genome. Mammalian pregnancy, which features gametes and embryos expressing proteins derived from disparate maternal and paternal genes, presents a serious challenge to the maternal immune system. A combination of strategies is involved in negotiating and preventing fetal immune rejection. Specific aspects of the immune response are circumvented, while others are engaged and persuaded to permit the intimate associations between maternal and fetal cells that characterize embryo development, implantation, and placental development. In this chapter, features of the female reproductive organs and specific strategies that protect against immune-mediated embryonic loss are described. Key concepts are that both the mother and the fetus contribute to successful pregnancy, that multiple immune evasion and tolerogenic mechanisms are deployed, and that immune adaptations to permit internalization of the fetus depend on ovarian steroid hormones and are initiated from the time of conception. Major features include (1) specific structural elements of the reproductive tissues that support immune accommodation, (2) characteristic features of resident leukocyte subpopulations, (3) immune suppressive and pro-tolerogenic molecules produced by maternal and fetal cells, (4) restricted fetal trophoblast expression of major histocompatibility molecules, and (5) a critical role for T-regulatory cells which suppress inflammation and inhibit maternal immunity to the fetus.

Following Hurricane Katrina, hundreds of thousands of Gulf Coast residents, many of whom were African Americans, were relocated to communities around the U.S., including predominantly European American communities. The experiences of relocation community members are a potent reminder of the ever-changing and multiply-determined nature of stereotypes and prejudice. Prejudice and stereotyping are dynamic processes that evolve in response to numerous factors. Although several models of prejudice and stereotyping are dynamic and/or time-oriented, research on these topics predominantly has involved single-session laboratory experiments examining single mechanisms. To illustrate the importance of dynamic research, we outline a longitudinal model describing the potential independent and interactive effects of inter group contact, individuating information, and perceived group threat from Katrina evacuees on prejudice and stereotypes about African Americans. The "natural experiment" created by the Katrina relocation reminds psychologists that naturalistic and longitudinal research on prejudice and stereotypes is an important complement to laboratory experiments.

Study Objectives. To examine potential threats to internal and external study validity caused by differential patient withdrawal from a randomized controlled trial evaluating pharmacist management of hypertension, to compare the characteristics of patients who withdrew with those of patients who completed the study, and to identify characteristics that predispose patients to withdraw from hypertension management. Design. Prospective, randomized, comparative study. Setting. Network of primary care clinics. Patients. Four hundred sixty-three patients with a diagnosis of hypertension and a last documented systolic blood pressure of 160 mm Hg or greater and/or diastolic blood pressure of 100 mm Hg or greater. Intervention. Patients were randomly allocated to the pharmacist intervention or usual-care (control) group. Those in the pharmacist intervention group were collaboratively managed by a primary care clinical pharmacy specialist and their primary care provider. Patients in the control group received usual care from only their primary care provider. Measurements and Main Results. Of the 463 patients, 191 (41%) withdrew from the study after randomization and 272 (59%) completed the study. Patients who withdrew from the pharmacist intervention group were similar to patients who withdrew from the usual-care group with respect to age, sex, insurance status, and chronic conditions. Patients who smoked or had commercial insurance were more likely to withdraw from the study than the other participants. However, multivariate analysis of all variables, when adjusted for the effect of the intervention, revealed that insurance status was the only variable associated with a heightened probability of withdrawal (p=0.002). Conclusion. Although this study had a high withdrawal rate, between-group patient characteristics remained balanced. Therefore, internal validity was preserved, and outcomes from the study groups could be reliably compared. A lack of significant differences between patients who withdrew versus those who completed, with the exception of insurance status, suggests that external validity was not jeopardized.

In response to growth factors, vascular smooth muscle cells (VSMCs) undergo a phenotypic modulation from a contractile, non-proliferative state to an activated, migratory state. This transition is characterized by changes in their gene expression profile, particularly by a significant down-regulation of contractile proteins. Platelet-derived growth factor (PDGF)-BB has long been known to initiate VSMC de-differentiation and mitogenesis. Insulin-like growth factor (IGF)-I, on the other hand, has differing effects depending on the model studied. Here, we report that both IGF-I and PDGF-BB stimulated VSMC de-differentiation of rat heart-derived SMCs in culture, although only PDGF-BB was capable of inducing proliferation. Although both PDGF-BB and IGF-I stimulation resulted in decreased smooth muscle alpha-actin expression and increased matrix metalloproteinase (MMP)-2 expression, the response to IGF-I was significantly more rapid. The increased MMP-2 expression in response to both growth factors was due to increased transcription rates and was dependent on the action of phosphatidylinositol 3-kinase (PI3K) and its downstream effector, Akt. Both PDGF-BB and IGF-I activated PI3K/Akt to similar degrees; however, only PDGF-BB concomitantly stimulated an inhibitory signaling pathway that antagonized the effects of Akt but did not alter the extent or duration of Akt activation. Together, these findings suggest that changes in MMP-2 expression are part of the program of VSMC phenotypic modulation and that both PDGF-BB and IGF-I, despite their different abilities to induce proliferation in this model, are capable of inducing VSMC activation.

Human leukocyte antigen-G (HLA-G) is a major histocompatibility complex class Ib gene expressed in normal organs and in some tumors. The glycoproteins encoded by this gene are best known for their immunosuppressive properties. Because isoform-specific expression of HLA-G in male reproductive organs has not been reported, we investigated HLA-G1, -G2, -G5, -G6 mRNAs and proteins in four-to-five samples of normal prostate glands, prostates with benign prostatic hyperplasia and prostate adenocarcinomas using RT-PCR and immunohistochemistry. All tissues contained HLA-G1, -G2, -G5 and -G6 specific mRNAs, but only HLA-G5 protein was detectable. In normal prostate glands, HLA-G5 protein was prominent in the cytoplasm of tubuloglandular epithelia and in glandular secretions. Staining was reduced in samples of benign prostatic hyperplasia but remained localized to the cytoplasm of glandular epithelia and secretions. In prostatic adenocarcinomas, HLA-G5 protein was detectable mainly in the secretions. Thus, HLA-G5 but not HLA-G1, -G2 or -G6 is produced in the normal prostate and is present in prostatic secretions. In addition, normal cellular localization is disturbed in benign and malignant prostatic adenocarcinomas. The results are consistent with this molecule may influencing female immune receptivity to sperm and suggest that such immunosuppression could be disturbed in men with prostatic adenocarcinomas.

The apoptosis cascade that plays a central role in normal and pathological processes is strictly controlled, in part by FLIP (Fas-associated death domain-like interleukin-1beta-converting enzyme-inhibitory protein), an inhibitor of caspase-8. Here, we report the expression of long and short isoforms of FLIP mRNAs and proteins in early and late gestation human placentas, term cytotrophoblast cells and two choriocarcinoma cell lines, JEG-3 and Jar. Reverse transcriptase polymerase chain reaction identified mRNAs derived from the FLIP gene in all samples. Analysis by immunoblotting revealed that both long and short forms of FLIP proteins are present in early and late gestation human placentas with increasing levels over gestation and that FLIP proteins are present in normal and transformed trophoblast cells. Immunohistochemical experiments performed on paraformaldehyde-fixed tissue sections taken from early and late stages of pregnancy demonstrated that FLIP proteins are present in caspase-8-expressing cells and that expression patterns of FLIP differed according to cell lineage and stage of cell differentiation. The results of this study are consistent with the postulate that FLIP proteins have critical roles in placental cell survival and suggest that FLIP may protect normal and transformed trophoblast cells from cell death.

Modulation of the maternal immune system by the placenta is a mechanism by which the fetus ensures its own survival in a genetically foreign environment. The immunoinhibitor CD274 (also called B7-H1 or PD-L1) is highly expressed in the placenta, positioned to interact with maternal leukocytes. Further, immunoblot analysis of first- and second-trimester placental lysates showed that CD274 expression is low in the first trimester but dramatically rises around the onset of the second trimester. As this coincides with the expected onset of maternal blood flow to the placenta and a corresponding rise in local oxygen tension, we explored the possibility that oxygen regulates CD274 expression in trophoblast cells by culturing term trophoblast cells under oxygen concentrations similar to those found in vivo. Indeed, CD274 protein levels paralleled the in vivo situation: expression increased with rising oxygen concentrations. Furthermore, downregulation of CD274 mRNA by low oxygen was rapid, occurring within 4–12 h. We conclude that oxygen is a potential mediator of CD274 expression in vivo such that it is induced coincidentally on exposure of fetal tissues to maternal blood. Further, the regulation of this immunomodulator by oxygen may implicate its alteration during and involvement in the pathogenesis of complications of pregnancy such as preeclampsia.

Neonatal ethanol exposure in animals results in performance deficits on tests of hippocampus-dependent spatial memory, and recent studies have shown that extra dietary choline can ameliorate some of these impairments. In this experiment, rats were administered 5.25 g/kg ig ethanol per day or sham intubations on Postnatal Days (PD) 4-9 and choline (0.1 ml of an 18.8 mg/ml solution) or saline subcutaneously on PD 4-20. On PD 30, rats were given delay or trace fear conditioning trials and were tested for conditioned stimulus-elicited freezing 24 hr later. Neonatal ethanol produced a profound impairment in trace conditioning that was reversed by choline. Groups did not differ in delay conditioned responding, indicating that neonatal ethanol produces a relatively selective cognitive deficit that can be alleviated with supplemental choline.

In two experiments with rats, we examined the developmental emergence of conditioned freezing following trace and short-delay conditioning and also included a long-delay comparison group. In the short-delay and trace groups, a 10-sec conditioned stimulus (CS) was paired with shock; for the trace rats, a 10-sec trace interval followed CS termination. The long-delay groups received a 20-sec CS paired with shock, to equate for the longer interstimulus interval (ISI) in the trace group. Trace conditioning emerged later in development than did short-delay conditioning (see Moye & Rudy, 1987). Importantly, long-delay conditioning emerged in parallel with trace conditioning, at a similar time, and with similar strength. These findings suggest a role for the longer ISI, as opposed to the unfilled gap per se, in the late emergence of trace conditioning. The role of the hippocampus in trace conditioning and the possibility that young rats encode the temporal relationship between CSs and unconditioned stimuli are also considered.

Human prenatal ethanol exposure that occurs during a period of increased synaptogenesis known as the “brain growth spurt” has been associated with significant impairments in attention, learning, and memory. The present experiment assessed whether administration of ethanol during the brain growth spurt in the rat, which occurs shortly after birth, disrupts attentional performance. Rats were administered 5.25 g/kg/day ethanol via intragastric intubation from postnatal days (PD) 4–9, sham-intubation, or no intubation (naive). Beginning at PD 90, animals were trained to asymptotic performance in a two-lever attention task that required discrimination of brief visual signals from trials with no signal presentation. Finally, manipulations of background noise and inter-trial interval duration were conducted. Early postnatal ethanol administration did not differentially affect acquisition of the attention task. However, after rats were trained to asymptotic performance levels, those previously exposed to ethanol demonstrated a deficit in detection of signals but not of non-signals compared to sham-intubated and naive rats. The signal detection deficit persisted whenever these animals were re-trained in the standard task, but further task manipulations failed to interact with ethanol pretreatment. The present data support the hypothesis that early postnatal ethanol administration disrupts aspects of attentional processing in the rat.

To investigate the correlations between observed clinical morbidity and dosimetric parameters for whole pelvic radiotherapy (WPRT) for prostate cancer using either three-dimensional conformal radiotherapy (3D-CRT) or intensity-modulated radiotherapy (IMRT).Between December 1996 and January 2002, 27 patients with prostate adenocarcinoma were treated with conformal WPRT as part of their definitive treatment. WPRT was delivered with 3D-CRT in 14 patients and with IMRT in 13 patients. For each of the patients treated with IMRT, optimized conventional two-dimensional (2D) and 3D-CRT plans were retrospectively generated for the whole pelvic phase of the treatment. Dose-volume histograms for the bowel, bladder, and rectum were compared for the three techniques. Acute toxicities were evaluated for all 27 patients, and late toxicities were evaluated for 25 patients with sufficient follow-up. Toxicities were scored according to the Radiation Therapy Oncology Group morbidity grading scales. Median follow-up was 30 months.Three-dimensional-CRT resulted in a 40% relative reduction (p0.001) in the volume of bowel receiving 45 Gy compared with 2D, and IMRT provided a further 60% reduction relative to 3D-CRT (p0.001). Compared with either 2D or 3D-CRT, IMRT reduced the volume of rectum receiving 45 Gy by 90% (p0.001). Overall, 9 patients (33%) experienced acute Grade 2 gastrointestinal (GI) toxicity, and only 1 of these patients was treated with IMRT. Antidiarrhea medication was required for 6 patients (22%). However, 5 of these 6 patients also received chemotherapy, and none were treated with IMRT. No Grade 3 or higher acute or late GI toxicities were observed. No cases of late radiation enteritis were observed. Acute and late genitourinary toxicity did not appear significantly increased by the addition of conformal WPRT.Compared to conventional 2D planning, conformal planning for WPRT resulted in significant reductions in the doses delivered to the bowel, rectum, and bladder. IMRT was superior to 3D-CRT in limiting the volume of bowel and rectum within high-dose regions. These dosimetric findings correlated with low rates of acute and late GI morbidity.

The rhizosphere competence of the biological control agent Trichoderma atroviride isolate C52 was studied on onion roots both in the glasshouse and in the field when introduced into soil in a range of formulations. Proliferation of T. atroviride in the rhizosphere was formulation-dependent. A pellet formulation maintained the fungal concentration at 105 cfu per g soil, whereas solid-substrate and seed-coating formulations gave concentrations of 104 and 101 cfu per g soil, respectively. To facilitate rhizosphere-competence studies, a UP-PCR band profile generated with primer L45 for isolate C52 was used to enable conclusive identification of T. atroviride C52 when recovered from soil. When isolate C52 was introduced into Sclerotium cepivorum-infested soil as both pellet and solid-substrate formulations, there was no statistically significant difference in the disease control between these treatments, but the pellet treatment doubled the percentage of healthy plants compared with the control treatment.

Maternal antigen presenting cells, which are macrophages and dendritic cells, are scattered throughout human decidualized endometrium during all stages of pregnancy. These powerful, multi-functional leukocytes reside in close proximity to uterine glandular epithelium, uterine blood vessels, and HLA-G-producing invasive cytotrophoblast cells. Macrophages and dendritic cells, which express the HLA-G receptors, ILT2 and ILT4, play major roles in driving innate and adaptive immune responses, altering the behavior of local stromal cells, shaping the cytokine microenvironment, and protecting the tissue from infection. Therefore, encounters between decidual antigen presenting cells and HLA-G molecules are likely to influence uterine and placental homeostasis as well as local maternal immune responses to the fetus during pregnancy.

Context: The premise of the problem-oriented medical record is that an accurately defined problem list will directly result in more thorough and efficient patient care. However, little empirical evidence exists demonstrating improved patient outcomes as a result of an adequately structured problem list. Objective: To determine the impact of problem list documentation of heart failure on the likelihood that evidence-based pharmacotherapy has been prescribed. Design: Cross-sectional study. Setting: Community-based primary care clinics in Portland, Oregon. Subjects: Active patients in the network with a left ventricular ejection fraction of 40% or less, with and without heart failure, in their structured problem list. Main Outcome Measures: The proportion of patients prescribed medications with known benefits for systolic dysfunction. Results: In this group of patients with known systolic dysfunction, the likelihood of therapy with either an angiotensin converting enzyme inhibitor or angiotensin II receptor blocker was higher in patients who had heart failure listed on their problem list compared to patients who did not (92.2% vs 76.7%; P

Objective To determine the validity of an electronic health record (EHR) in the identification of patients with left ventricular dysfunction in a primary care setting. Design A cross-sectional study. Setting Nine clinics participating from the Providence Research Network (PRN) comprising 75 physicians serving approximately 200 000 patients. All clinics utilise the Logician™ EHR for all patient care activities. Patients The study included all PRN patients with an active chart. Interventions All patients with a heart failure diagnosis in the problem list were identified by database query. Left ventricular ejection fraction (LVEF) data were identified through query of local cardiology and hospital echocardiography databases. Additional LVEF data were sought in a manual search of paper charts. Measurements and main results To determine the problem list coding accuracy for a heart failure (HF) diagnosis we evaluated sensitivity, positive predictive value and related derived statistical measures using documented LVEF as the ‘gold standard’.Of 205 755 active PRN patients, 1731 were identified with a problem list entry of HF. Based on comparison with documented LVEF, the sensitivity for problem list entry was 43.9% and 54.4% when HF was defined as an LVEF ≤55% and ≤40%, respectively. Conclusion The validity of an EHR problem list entry of HF was poor. The problem list validity could be enhanced through reconciliation with other data sources. Inaccurate EHR problem lists may have clinical consequences, including underprescribing of beneficial therapies.

Background: Recent studies have revealed that the adolescent brain may be especially vulnerable to ethanol-induced toxicity. Corticolimbic regions are more severely damaged following ethanol exposure during adolescence than during adulthood. The consequences of adolescent ethanol exposure on cognition however, have only recently begun to be explored. Methods: Male and female rats were administered 0, 1.5, 2.5 or 4.5 g/kg ethanol (20% v/v) by acute intragastric gavage during adolescence (postnatal days [PD] 28, 30, 32 and 34). On PD 40, half of the subjects in each dose group were given 5 pairings of a 10-sec flashing light (CS; conditioned stimulus) immediately followed by mild footshock (US; unconditioned stimulus), a procedure known as delay conditioning. The other half were also given 5 CS-US pairings, but the US was presented 10 sec after CS offset, a procedure known as trace conditioning. All subjects were tested for CS-elicited freezing 24 h later. Results: There was no effect of adolescent ethanol exposure on delay conditioned responding, with all subjects demonstrating comparable levels of CS-elicited freezing. In contrast, the amount of freezing in the trace conditioned subjects was negatively correlated with prior ethanol dose. Specifically, exposure to 2.5 or 4.5 g/kg during adolescence resulted in a deficit in trace conditioned responding. Conclusions: These data indicate that intermittent exposure to ethanol during adolescence results in impairment in hippocampal-dependent trace conditioning that persists beyond the period of ethanol exposure. Delay conditioning was unaffected by prior ethanol treatment, indicating that there was no difficulty in detecting the CS or US, or in the ability to engage in freezing behavior. These results suggest that the adolescent brain may be particularly vulnerable to the effects of repeated exposure to ethanol that can have consequences for nonspatial, hippocampal-dependent cognitive abilities.

The rate of habituation of the heart rate orienting response (OR) to both olfactory and auditory cues was examined in subjects neonatally exposed to ethanol. Subjects were administered 5.25 g/kg/day ethanol in a binge-like manner on either postnatal days (PDs) 4-9 or 10-15. Controls were given sham intubations. Ethanol exposure had no effect on the form or magnitude of the heart rate OR; all subjects exhibited a monophasic bradycardia to the olfactory and auditory cues. However, ethanol treatment severely affected the rate of response habituation to an olfactory cue, regardless of the timing of that exposure. Ethanol treatment on either PDs 4-9 or 10-15 resulted in a failure of animals to show complete heart rate response habituation. For the auditory cue, rate of response habituation was unaffected by ethanol treatment. These results indicate that response habituation deficits in a postnatal model of human fetal-alcohol exposure are modality specific. The response habituation paradigm may be useful for studying mechanisms of short-term recognition memory deficits following early alcohol exposure, although modality of the test stimulus needs to be taken into account. The results suggest that central nervous system structures affected by ethanol may be specific to those involved in olfactory processing, such as the olfactory bulb or hippocampus.

Plasma formed in the immediate vicinity of a meteoroid as it descends through Earth's atmosphere enables high-gain radars such as those found at Kwajalein, Arecibo, and Jicamarca to detect ablating meteoroids. In the work presented here, we show that these head echo measurements preferentially detect more energetic meteoroids over less energetic ones and present a method of estimating the effects of this bias when measuring the velocity distributions. To do this, we apply ablation and ionization models to estimate a meteoroid's plasma production rate based on its initial kinetic energy and ionization efficiency. This analysis demonstrates that, almost regardless of the assumptions made, high-gain radars will preferentially detect faster and more massive meteoroids. Following the model used by Taylor (1995, Icarus 116, 154–158), we estimate the biases and then apply them to observed meteoroid velocity distributions. We apply this technique to observations of the North Apex meteoroid source made by the Advanced Research Project Agency Long Range Tracking and Instrumentation Radar (ALTAIR) at two frequencies (160 and 422 MHz) and compare results from the Harvard Radio Meteor Project (HRMP) at High Frequency (HF, 40.9 MHz). Both studies observe a peak in the distribution of North Apex meteoroids at approximately 56 km s −1 . After correcting for biases using Taylor's method, the results suggest that the mass-weighted peak of the distribution lies near 20 km s −1 for both studies. We attribute these similarities to the fact that both radar systems depend upon similar ablation and ionization processes and thus have a common mass scale.

Large-aperture radars detect the high-density plasma that forms in the vicinity of a meteoroid and moves approximately at its velocity; reflections from these plasmas are called head echoes. To determine the head plasma density and configuration, we model the interaction of a radar wave with the plasma without using assumptions about plasma density. This paper presents a scattering method that enables us to convert measurements of radar cross-section (RCS) from a head echo into plasma density by applying a spherical scattering model. We use three methods to validate our model. First, we compare the maximum plasma densities determined from the spherical solution using 30 head echoes detected simultaneously at VHF and UHF. Second, we use a head echo detected simultaneously at VHF, UHF and L-band to compare plasma densities at all frequencies. Finally, we apply our spherical solution to 723 VHF head echoes and calculate plasma density, line density and meteoroid mass in order to compare these values with those obtained from a meteoroid ablation and ionization model. In all three comparisons, our results show that the spherical solution produces consistent results across a wide frequency range and agrees well with the single-body ablation model.

This study investigated effects of birth weight and postnatal nutrition on organ growth in neonatal lambs. Suffolk x (Finnsheep x Dorset) low- (mean +/- SD 2.29 +/- 0.34 kg, n = 28) and high- (4.84 +/- 0.45 kg, n = 20) birth-weight male lambs were studied. Lambs within each birth weight category were allocated to be individually grown rapidly (ad libitum fed, ADG 337 g, n = 20) or slowly (ADG 150 g, n = 20) on a liquid diet to live weights up to approximately 20 kg. All organs weighed less at birth in small than in large newborns (P < 0.001), except the adrenals (P = 0.10). At birth, as a percentage of empty body weight (EBW), small newborns had larger testes (0.14 vs. 0.10%, P = 0.023) and smaller thymus (0.17 vs. 0.37%, P = 0.009), and tended to have a larger heart (0.85 vs. 0.75%, P = 0.060) and a smaller spleen (0.10 vs. 0.14%, P = 0.054) than large newborns. During the first 2 to 3 wk postpartum, small newborns had greater fractional growth rates of organs than large newborns, most notably spleen, thymus, and liver. Postnatal growth of organs was more closely associated with EBW than age, except for lungs, testes, and stomach. At completion of rearing to 20 kg of live weight, small newborns had a spleen approximately 30% heavier than large newborns (P < 0.001). Testes weights were 37% and 24% greater in small newborns reared slowly and rapidly, respectively, compared with their high-birth-weight counterparts (P = 0.034). It was also evident that postnatal nutrition altered the mass of individual organs at the conclusion of the rearing period without affecting the combined weight of dissected organs. Slowly reared lambs had a larger pancreas (+27%, P = 0.002), stomach complex (+83%, P < 0.001), large intestine (+39%, P < 0.001), entire gastrointestinal tract (+18%, P = 0.002), and testes (+54%, P = 0.016) and tended to have a larger heart (+6%, P = 0.068) than their rapidly reared counterparts at 20 kg of live weight. Rapidly reared lambs had a larger thymus (+61%, P = 0.003), liver (+34%, P < 0.001), kidneys (+33%, P < 0.001), and small intestine (+17%, P < 0.001) and tended to have a larger thyroid (+13%, P = 0.054) at 20 kg of live weight than slowly reared lambs. The functional significance of the smaller thymus at birth and increase in spleen and testes weights at 20 kg of live weight in low- compared with high-birth-weight lambs warrants further investigation. It also remains to be established whether these differences at 20 kg of live weight persist.

With the introduction and marketing of sildenafil, national attention has focused on sexual dysfunction in men. However, strides are being made to focus more effort on evaluation and treatment of female sexual disorder (FSD) since a 1999 national survey reported prevalence rates as high as 43% in women. Evaluation and assessment of FSD requires a comprehensive history and physical examination. Understanding of FSD requires knowledge and understanding of sexual anatomy, physiology, and pathophysiology. Nonpharmacologic treatment may include sex therapy or relationship counseling. Pharmacologic treatment of FSD has focused mainly on hormonal therapies, including estrogen and testosterone, with the majority of studies being done in postmenopausal and hysterectomized women. However, recent studies indicate a more prudent and careful use of hormonal therapies. Research is also moving toward evaluation of bupropion and sildenafil as options for women who suffer from FSD. Available evidence for treatment alternatives is scant and in some cases inconclusive.

The human placenta utilizes both active and passive mechanisms to evade rejection by the maternal immune system. We investigated the pattern of expression of the B7 family of immunomodulatory molecules B7-H1 (PD-L1), B7-2 (CD86), and B7-1 (CD80) at the term maternal-fetal interface. Northern blot and reverse transcription-polymerase chain reaction (RT-PCR) analyses showed that B7-H1 mRNA is abundant in term placenta and that cytotrophoblasts are sources of this message. Immunohistochemistry demonstrated that B7-H1 is constitutively expressed by the syncytiotrophoblast and by extravillous cytotrophoblasts, both of which are juxtaposed to maternal blood and tissue. By contrast, placental stromal cells, including macrophages, lacked the protein. Expression of B7-H1 protein was low in first-trimester placenta compared to second- and third-trimester tissue (P < 0.05) and was enhanced in cultured cytotrophoblasts by treatment with either interferon-gamma or epidermal growth factor (P < 0.05), suggesting that one or both of these mediators regulates B7-H1 expression in the placenta. RT-PCR and immunofluorescence analysis of term placental tissue revealed different patterns of expression of the immunostimulatory protein, B7-2. In contrast to B7-H1, B7-2 mRNA and protein were absent in cytotrophoblast cells but present in maternal macrophages and some fetal macrophages. The B7-1 mRNA and protein were absent at the maternal-fetal interface. These studies document expression of the B7 family proteins at the maternal-fetal interface and demonstrate that B7-H1 is positioned such that it could facilitate protection of fetal cells against activated maternal leukocytes. Conversely, B7-2 was absent on trophoblasts and was appropriately localized to fetal and maternal macrophages, which may participate in antigen presentation.

Root pathogens and rust diseases can cause extensive damage to Canadian forest tree species. Loss of growth, although difficult to visualize, is substantial over the life of a tree. Understanding host–pathogen interactions is important in managing yield loss and can aid in the identification of disease-resistant trees. However, studying the interactions involving forest pathogens offers both challenges and opportunities. Some of these issues are described from our perspective through working on white pines – blister rust pathosystem and pathogens that infect the roots. Several resistance mechanisms to the white pine blister rust fungus, Cronartium ribicola, have been identified in pine. At the molecular level, several defence responsive proteins and their genes have been characterized. Some of these are identified to be potential candidates for markers associated with resistance or susceptibility. The expression and function of the human major histocompatibility complex (MHC) class Ia genes, human leukocyte antigen (HLA)-A, -B, and -C, is well-established; they are expressed in most nucleated cells and present endogenous peptides to CD8+ T cells. However, MHC class Ib genes are poorly characterized and have unknown functions. In humans, the best-characterized class Ib gene is HLA-G. This gene has a restricted tissue expression of the mRNA and a unique pattern of protein expression; it is expressed mainly in the extravillous cytotrophoblast cells in the placenta. The function of HLA-G is not clear, but its presence at the maternal-fetal interface suggests a role in protection of the semiallogeneic fetus. Whereas functional studies using in vitro models and transgenic mice provide useful insights regarding the potential function of this molecule, in vivo studies cannot be performed in humans. This study investigated effects of birth weight and postnatal nutrition on regulation of energy metabolism in the neonatal lamb. Low (mean +/- SD 2.289 +/- 0.341 kg, n = 28) and high (4.840 +/- 0.446 kg, n = 20) birth weight male Suffolk x (Finnsheep x Dorset) lambs were individually reared on a liquid diet to grow rapidly (ad libitum fed, ADG = 337 g, n = 20) or slowly (ADG = 150 g, n = 20) from birth to live weights (LW) up to approximately 20 kg. At birth, small newborns had higher plasma concentrations of urea nitrogen (mean +/- SEM 8.31 +/- 0.25 vs 6.39 +/- 0.32 mM, P = 0.002) and somatotropin (ST, 49.1 +/- 17.0 vs 10.8 +/- 4.3 ng/mL, P = .045) and lower IGF-I (36.1 +/- 6.8 vs 157.7 +/- 21.8 ng/mL, P < 0.001) than large newborns. Plasma glucose (1.42 +/- 0.23 vs 2.63 +/- 0.95 mM, P = 0.147) and insulin (0.09 +/- 0.02 vs 0.13 +/- 0.06 ng/mL, P = 0.264) concentrations did not differ. Urea nitrogen concentration in plasma peaked and then declined rapidly in all lambs during the first week postpartum, and plasma ST declined on a body-weight-related basis from birth. During rearing to 20 kg LW, plasma insulin was higher in low- vs high-birth-weight lambs. Lambs fed ad libitum had greater plasma concentrations of glucose, urea nitrogen, insulin, and IGF-I compared to those fed a restricted diet (ADG = 150 g). The results suggest that during the early postpartum period, newborn lambs exhibit the fetal characteristic of high rates of amino acid oxidation. The results also support the notion that, at birth, low-birth-weight lambs are less mature than high-birth-weight lambs in aspects of metabolic and endocrine development, which may enhance their capacity to utilize amino acids for energy production and to support gluconeogenesis during the immediate postpartum period. Being small at birth also resulted in elevated plasma insulin concentrations when adequate nutriment to support moderate or rapid growth was provided postpartum, although it remains to be elucidated whether this more chronic effect persists in the longer term.
