Abstract Background East and South East Asian subjects as well as Amerindians and Hispanic subjects are predominantly affected by Vogt-Koyanagi-Harada disease. In Europe, only few studies have described the clinical features and treatment of this disease, especially in France. Methods This retrospective case series was based on data collected from patients with a VKH disease diagnosed from January 2000 to March 2017, provided by three French Tertiary Centers. Results Forty-one patients (16 men and 25 women) were diagnosed: average age at diagnosis was 38.7 years. Patients were mainly from Maghreb (58%), but ethnic origins were multiple. Pleiocytosis was observed in 19 cases (63%) and 17 out of 41 patients showed audio vestibular signs (41%), and 11 showed skin signs (27%). Thirty-four were treated with corticosteroids (83%), 11 with an immunosuppressant treatment (27%) and 5 with biological therapy drugs (13%). Relapse was observed in 41% patients, even though final average visual acuity had improved. We did not find any significant clinical difference in the population from Maghreb compared to other populations, but for age and sex trends, since there was a majority of younger women. Conclusion We report here the second largest French cohort reported to date to our knowledge. The multiethnicity in our study suggests that VKH disease should be evoked whatever patients’ ethnicity.

The analysis of the vertical movements of the soil in the Po River plane of the Emilia-Romagna Region (Italy) was updated through an interferometric analysis referred to the 2011–2016 time-span. This activity is a continuation of previous studies on the state of knowledge of vertical soil movements in the same area, analyzed firstly by levelling and GNSS and more recently by SAR interferometry for the periods 1992–2000, 2002–2006, 2006–2011, on behalf of the Emilia-Romagna Region. The survey area analysed was approximately 13 300 km2, which corresponds to the territory of the regional plain. The interferometric dataset was calibrated through the use of velocity time series of several permanent GNSS stations. Among the 36 stations analysed, 22 were included in the study area: 16 were used for the calibration and 6 as check points). The velocities required for the calibration of the SAR analysis were calculated in the period following the important seismic events that struck the territory of the Emilia Romagna Region in May 2012. The interferometric analysis was carried out by TRE ALTAMIRA using the SqueeSAR™ technology. In particular, in order to update the interferometric dataset to 2016, it was necessary to perform a joint processing of the available RADARSAT-1 data and of the data acquired by the RADARSAT-2 satellite using a specific operating mode of the SqueeSAR™ algorithm known as stitching; this approach allowed the joint processing of images acquired in the same geometry by these two satellites. The study of the time series of the GNSS permanent stations used to provide the velocity datum to the interferometric analysis, is described, and the results of the SqueeSAR™ interferometric processing are reported. Statistical analyses on the spatial distribution and the type of scatterers have been performed during the screening and validation procedures of the dataset, and for the identification and removal of the outliers. Finally, the resulting map is described in order to analyse the measured soil movements with respect to the results obtained in past analyses, and the possible geological and human-induced causes, which could have produced them.

Multiple processes contributing to natural land subsidence in a shallow coastal aquifer near Ravenna (Italy) were identified by analysing the relationships among different data set time series (water table level, rainfall, land reclamation drainage, sea level, etc.) and establishing the correlations with vertical ground motion observed at a high-resolution settlement gauge. Our study highlights the presence of three deformation components related to different processes controlling land subsidence: elastic, delayed-elastic, and irreversible (plastic) components. The elastic and delayed-elastic components are closely related to water table fluctuations that change the effective stress in two portions of the coastal aquifer at a daily (in the sandy unconfined portion) and seasonal time scales (in the layered clay-rich semi-confined prodelta portion), respectively. The irreversible component represents the trend in the land subsidence time series and is due to primary consolidation (pore pressure dissipation) of the fine-grained prodelta levels above where the settlement gauge is located. The amplitudes of the elastic component can be up to 0.2–0.3 mm whereas the amplitude of the delayed-elastic component reaches 0.89 mm. The primary consolidation rate of deformation is 0.9 mm yr−1 and constrains the likely age of prodelta sediments deposition to 1300–2800 years before present. The delayed-elastic subsidence rate has similar magnitude to that due to primary consolidation and is connected to poroelastic effects in the prodelta sequence following seasonal variations in water table. Our findings are important for planning land subsidence management and monitoring strategies especially where the surface aquifer structure is heterogeneous due to different depositional settings. The natural land subsidence rate in the Holocene sediments of the shallow coastal aquifer of Ravenna (North eastern Italy) that we measured in this study accounts for 10 %–20 % of the total current land subsidence rate observed in this portion of Ravenna coastal area (10–20 mm yr−1).

Guidelines recommend to explore mild delayed hypersensitivity reactions to beta-lactams (BL) in children with direct drug provocation tests (DPTs). The optimal duration of these DPTs remains unknown and DPTs lasting 1 to 10 days are proposed. No study has directly compared the frequency of positivity of directs DPTs according to their duration. To compare the frequency of positivity of DPTs of different duration performed to explore mild delayed hypersensitivity reactions to beta-lactams (BL) in children in different centers. Data from children explored between January 2019 and December 2021 for delayed mild reactions (isolated urticaria, maculopapular exanthema or non-specific skin rashes) to oral BL in six different pediatric centers performing DPTs of varying duration (1 day or 3 days or 2 days more than the index reaction) were analysed. In total, 673 children were included. The duration of the DPTs were 1 day in 60 cases (8.9%), 3 days in 69 cases (10.3%), and 2 days more than the index reaction in 544 cases (80.8%). The median age at the index reaction was 1.7 years [0.8–3.7] in the "group 1 day", 1.9 [1–3.6] years in the group "3 days" and 1.5 [1–3.3] in the group "2 days more". Amoxicillin was the main BL involved (72.8%). The median age at workup was 4.6 years [2.7–8.7] in the "group 1 day", 3.8 years [1.8–8.3] in the "group 3 days" and 3 years [1.7–7] in the group "2 days more". General characteristics of patients in the 3 groups were similar. Fifty-eight DPTs were positive (8.6%); reactions occurring during DPTs were all mild and delayed isolated skin rashes resolving spontaneously or with H1-antihistamines. One DPT was positive in the "group 1 day" (1.5%), 4 (5.8%) in the "group 3 days", and 53 (9,7%) in the "group 2 days more than the index reaction" (P = 0.08). The frequency of positive DPTs was higher for long DPT ("2 days more") than short DPT (1 or 3 days) (3.9% vs. 9.7%, P < 0.05). L'objectif est la comparaison des performances diagnostiques de deux puces IgE multi-allergéniques de composition et de principe différents. Vingt et un patients polysensibilisés ou ayant présenté une anaphylaxie inexpliquée ont été testés par deux puces : la puce ISAC® et la puce ALEX2®. La concordance globale des résultats entre les deux puces est très bonne (97,8 %) pour les allergènes communs. L'analyse par famille moléculaire montre une plus grande fréquence de positivité d'ALEX® pour les LTP (7,4 % versus 5,8 %), alors que c'est l'inverse pour les PR-10 (14,9 % en ISAC® versus 11,9 % en ALEX®). Cependant, les concentrations observées ne sont pas comparables et sont plus élevées en ALEX® pour les PR-10. Comme attendu, le prétraitement par un inhibiteur spécifique bloque l'interférence des IgE anti-CCD dans la puce ALEX®. L'analyse par patient a permis de mettre en évidence des sensibilisations à certains allergènes moléculaires spécifiques de chaque puce : viandes, poissons et pistache présents uniquement sur ALEX2® ou alpha-Gal sur ISAC®. Les résultats obtenus avec les deux puces sont globalement concordants pour les allergènes communs. Une puce plus complète associant des extraits comme des allergènes moléculaires non disponibles pour certains en unitaires devrait trouver sa place dans l'exploration d'anaphylaxie inexpliquée, mais le choix et le nombre d'allergènes testés devrait évoluer pour répondre au mieux aux besoins cliniques. The aim is to compare the diagnostic performances of two multi-allergenic IgE microarrays of different composition and principle. Twenty-one patients with polysensitivity or unexplained anaphylaxis were tested by two microarrays: the ISAC® biochip and the ALEX2® biochip. The overall agreement of results between the two biochips was very good (97.8%), Analysis by molecular protein family shows a higher frequency of ALEX® positivity for LTP (7.4% versus 5.8 %), while the opposite is true for PR-10 (14.9% for ISAC® versus 11.9% for ALEX®). However, the concentrations observed in ALEX® for PR-10 are higher. As expected, a pretreatment by a specific inhibitor blocked the interference of anti-CCD IgE in ALEX® biochip. The analysis by patient allowed to highlight sensitizations to some specific components of each biochip: meat, fish, and pistachio present only on ALEX2® or alpha-Gal on ISAC®. The results obtained with the 2 biochips are generally consistent for common allergens. A more complete chip combining extracts as molecular allergens not available in individual tests should find its place in the exploration of unexplained anaphylaxis but the choice and number of allergens tested should evolve to best meet clinical needs. La graine de tournesol est un aliment avec un fort potentiel sensibilisant mais finalement peu d'allergies confirmées. Les modes de sensibilisation sont multiples, respiratoires, digestifs et cutanés. De nombreuses manifestations cliniques ont été décrites, les profils des patients sont également très variés. Des réactions à l'huile de tournesol sont possibles mais exceptionnelles. Des prick-tests avec l'aliment natif et la recherche d'IgE spécifiques confirmeront le diagnostic. La recherche d'allergènes moléculaires a permis d'identifier notamment une LTP et des albumines 2S. Nous rapportons une série de cas déclarés au Réseau d'Allergo-Vigilance® et faisons une revue de la littérature sur le sujet. Sunflower seed is a food with a high sensitising potential but few allergies have been confirmed. Sensitization modes are multiple, respiratory, digestive and cutaneous. Numerous clinical manifestations have been described, and patient profiles are also very various. Reactions to sunflower oil are possible but very rare. Prick tests with the native food and the search for specific IgE antibodies will confirm the diagnosis. The search for molecular allergens has allowed the identification of LTP and 2S albumin. Il reste encore de nombreuses voies à explorer, notamment l'intelligence artificielle et les objets connectés. This e-health corresponds to different areas, both for patients, for caregivers and for institutions. Each medical specialty is involved differently in each area. In allergology, teleconsultation has particularly raised since 2020, while tele-expertise is very little used. Social Medias are providers of a lot of information on allergies but without any control on the veracity of these. The different facets of allergology can be discovered and deepened online thanks to numerous sites and apps, with content written or checked by doctors. Abstract Background Postoperative pulmonary complications (PPC) may result in longer duration of in-hospital stay and even mortality. Both thoracic surgery and intraoperative mechanical ventilation settings add considerably to the risk of PPC. It is unclear if one-lung ventilation (OLV) for thoracic surgery with a strategy of intraoperative high positive end-expiratory pressure (PEEP) and recruitment maneuvers (RM) reduces PPC, compared to low PEEP without RM. Methods PROTHOR is an international, multicenter, randomized, controlled, assessor-blinded, two-arm trial initiated by investigators of the PROtective VEntilation NETwork. In total, 2378 patients will be randomly assigned to one of two different intraoperative mechanical ventilation strategies. Investigators screen patients aged 18 years or older, scheduled for open thoracic or video-assisted thoracoscopic surgery under general anesthesia requiring OLV, with a maximal body mass index of 35 kg/m2, and a planned duration of surgery of more than 60 min. Further, the expected duration of OLV shall be longer than two-lung ventilation, and lung separation is planned with a double lumen tube. Patients will be randomly assigned to PEEP of 10 cmH2O with lung RM, or PEEP of 5 cmH2O without RM. During two-lung ventilation tidal volume is set at 7 mL/kg predicted body weight and, during OLV, it will be decreased to 5 mL/kg. The occurrence of PPC will be recorded as a collapsed composite of single adverse pulmonary events and represents the primary endpoint. Discussion PROTHOR is the first randomized controlled trial in patients undergoing thoracic surgery with OLV that is adequately powered to compare the effects of intraoperative high PEEP with RM versus low PEEP without RM on PPC. The results of the PROTHOR trial will support anesthesiologists in their decision to set intraoperative PEEP during protective ventilation for OLV in thoracic surgery. The article describes the case of an uncommon multicentric Epstein-Barr virus-caused extranodal nasal-type natural killer T-cell lymphoma disguised as unilateral panuveitis, diagnosed with an anterior chamber tap. The patient, initially presenting with recalcitrant sinusitis and unilateral loss of vision, underwent clinical examination, which showed right peripheral facial nerve palsy, severe panuveitis of the left eye, and unsparing obstruction.

Summary: Background: Kawasaki-like syndrome (KLS) is rare in adults; one third of these patients are infected by Human immunodeficiency virus (HIV). Our study reports cases of KLS occurring in HIV-positive adults and reviews the literature to compare their characteristics with HIV-negative adults and children with Kawasaki disease (KD). Methods: Report of cases and review of the literature. Results: Amongst 20 cases reviewed, including 4 who were managed at our institution, KLS was associated with severe immunosuppression and a high HIV viral load. There was frequent co-infection by hepatitis viruses. Desquamation and liver abnormalities were more frequent in HIV-negative adults whereas headaches and gastrointestinal disorders more common in HIV-positive adults. Intravenous immunoglobulin was effective therapy. Relapse was more frequent among HIV-positive patients. No cardiovascular complications or deaths occurred. Conclusions: Advanced immunosuppression due to HIV may predispose to KLS. The differential diagnosis that must be considered includes drug hypersensitivity reactions and staphylococcal infections.
