The increase of dehydrochlorination rate was investigated at different reaction times and amount of KF-Al 2O 3.

Bazal hücreli karsinoma beyaz ırkta en sık gözlenen deri kanseridir. Bazal hücreli nevüs sendromu klinik olarak çok sayıda bazal hücreli karsinoma oluşumu ve çeşitli gelişimsel anomalilerle karakterizedir. Topikal 5-Fluorourasil ve kriyoterapi, bazal hücreli karsinomanın tedavisinde kullanılan yöntemler arasındadır. Burada topikal 5-Fluorourasil ve krioterapi kombinasyonu ile başarılı bir şekilde tedavi edilen multipl bazal hücreli karsinomalı bir olgu sunulmaktadır. Basal cell carcinoma is the most common skin cancer of the white population. Basal cell nevus syndrome is characterized by the development of multiple basal cell carcinomas and various developmental anomalies. Topical 5-fluorouracil and cryotherapy are the methods that can be used in the treatment of basal cell carcinomas. Herein we report a patient with multiple basal cell carcinomas who have been successfully treated with the combination of 5-fluorouracil and cryotherapy.

Saçlı deri psoriazisi tedavisinde topikal kortikosteroidler ve kalsipotriol losyon en çok tercih edilen ajanlardır. Genellikle etkinliği arttırmak ve yan etkileri azaltmak için kombinasyon tedavileri kullanılmaktadır. Çalışmamızda kalsipotriol losyon, mometazon furoat losyon ve her iki ajanın kombinasyonunun saçlı deri psoriazisindeki etki ve yan etkilerini karşılaştırmayı amaçladık. Hafif ve orta şiddetteki saçlı deri psoriazisi olan 45 hasta randomize olarak üç eşit gruba ayrıldı. Birinci gruba kalsipotriol losyon, ikinci gruba mometazon furoat losyon ve üçüncü gruba her iki losyon 4 hafta boyunca uygulattırıldı. Çalışma sonunda, her üç grupta da yayılım skorları, kaşıntı skorları, toplam klinik belirti skorları ve psoriazis saçlı deri şiddet indeksi değerlerinde başlangıca göre azalma kaydedildi (p0.05). Kombine grupta kalsipotriol grubuna göre daha az sayıda yan etki gözlendi. Sonuç olarak mometazon losyon ve kalsipotriol losyon saçlı deri psoriazisinde etkili olmakla birlikte, her iki ajan kombine olarak kullanıldığında, etkinliğinin daha fazla, yan etkilerinin ise daha az olduğu gözlenmiştir. Background: Topical corticosteroids and calcipotriol lotion are the most preferred agents for the treatment of scalp psoriasis. Since combination treatments increase effectiveness and decrease side effects, they are usually preferred for treatment of scalp psoriasis. The aim of our study was to compare the effectiveness and side effects of momethasone furoate lotion, calcipotriol lotion and combination of these two agents for the scalp psoriasis. Materials and Methods: Forty-five patients who had mild to moderate scalp psoriasis were randomly divided to three groups. Calcipotriol lotion for the first group, momethasone furoate lotion for the second group and combination of these two agents for the third group were used for four weeks. İzotretinoin ve asitretin vitamin A türevi sentetik retinoidler olup, iskelet sistemi üzerine yan etkilerinin olduğu bilinmektedir. Çalışmamızda oral sentetik retinoid kullanan hastaların kemik mineral yoğunluğu ölçümlerini yaparak osteoporoz riski taşıyıp taşımadıklarını tespit etmeyi amaçladık. Çalışmaya 0.5 mg/kg/gün dozunda 16’sı asitretin ve 3’ü izotretinoin alan toplam 19 hasta alındı. Tedavi öncesinde ve 6. ayın sonunda lomber vertebraların (L2-L4) ve sol kalçanın (femur boynu ve trokanterik bölge) kemik mineral yoğunluğu ölçümleri (g/cm2) Dual Energy X-Ray Absorbsiometri ile yapıldı. Tedavi öncesi ve sonrası kemik mineral yoğunluğu ölçümlerinde istatistiksel olarak anlamlı bir fark saptanmadı (p0.05). Sonuç olarak, düşük doz (0,5 mg/kg/gün) ve kısa dönem (6 ay) izotretinoin ve asitretinin kullanımının, lomber vertebra ve kalçada osteoporoz yapıcı etkisinin olmadığı bulundu. Background and design: Isotretinoin and acitretin, known as retinoids, are synthetic derivaties of vitamin A. They have been known to cause a variety of skeletal side effects. The aim of the study was to determine whether the patients receiving synthetic retinoids are under the risk of osteoporosis. Material and Method: Nineteen patients who received 0.5 mg/kg/day dose of acitretin (n16) and isotretinoin (n3) were included in this study. Bone mineral density (g/cm2) of left hip (neck, trochanter) and lumbar spine (L2-L4) were measured by Dual Energy X-Ray Absorptiometry before and at the sixth month of the therapy. Results: We didn‘t find statistically significant difference in bone mineral density (p>0.05). Conclusion: Short term (six months) isotretinoin and acitretin with a low dose of 0.5 mg/kg/day were not due to osteoporosis at the lumbar spine and the hip.

Seboreik dermatit'in kesin bir tedavisi bulunmamakta birlikte, hastalığın kontrolunde sikhkla topikal steroid ve antifungal ajanlar kullanilmaktadir. Son zamanlarda topikal metronidazolun antiinflamatuar etkilerinden dolayı tedavide bir seçenek olabileceği görüşü ileri sürülmektedir. Biz de çalışmamızda, seboreik dermatitte topikal metronidazolun (o/oO.75) etkisini değerlendirmeyi amaçladık. Seboreik dermatitli 40 hasta, randomize olarak iki gruba ayrıldı. İlk gruba topikal % 0.75 metronidazol jel ve ikinci gruba da topikal taşıyıcı jel (plasebo) günde iki kez uygulanması önerildi. Lezyonlar şiddet skoru He 0-2-4-6 ve 8. haftalarda değerlendirildi. Hastaların 33'u (plasebo grubunda 18, metronidazol grubunda 15 hasta) çalışmayı tamamlayabildi. Çalışmayı tamamlayamayan hastaların 6'sı yan etkiler nedeni ile tedaviyi bıraktı ve bu hastalardan 5'i metronidazol grubunda idi. İki grup arasında 4. hafta ve tedavi sonunda ortalama şiddet skorları arasında fark ol-madığı tespit edildi (p0.001). Bu çalismada, seboreik dermatit tedavisinde topikal metronidazolun plaseboya bir üstünlüğü olmadığı tespit edilmiştir. Background and design: There is no definitive treatment but topical steroid creams and antifungal agents, are commonly used to control seborrhoeic dermatitis. Recently it is suggested that topical metronidazole with its anti-inflamatory effects could be useful in seborrheic dermatitis. In this study we aimed to evaluate the effectiveness of topical 0.75 °/o metronidazole gel. Material and Method: Forty patients with seborrheic dermatitis were randomly assigned to two treatment groups. Patients in first group were applied topical 0.75 % metronidazole gel and second group placebo (vehicle gel). Lesions were evaluated with the severity score at 2nd-4th-6th-8th weeks. Results: Thirty-three patients completed the study ( 15 patients in the metronidazole group, 18 patients in the placebo group). One patients in the placebo group, five patients in the metronidazole group left the study because of the side effects. There is no statistically significant difference in the severity score between two groups. Conclusion: In the present study we concluded that the effect of the metronidazole °/o 0.75 gel in the treatment of seborrheic dermatitis was not superior to placebo.

Myiasis is an infestation of maggot flies, usually found in tropical and non-tropical regions such as Africa and South America. Although cases are reported after trips to the endemic regions, it should not be overlooked, as in our case, that they may appear especially in poor socioeconomic conditions and the individuals who have to live in the unsanitary environment. Here, we present a 7-year-old myiasis case in our region because it is rare but it can be easily treated when it is diagnosed and it causes unwanted complications if it is not treated. Amaç: Eritrodermi, vücudun tam veya tama yakın kısmının eritem ve deskuamasyonu ile karakterize bir durumdur ve bir çok dermatozun seyri sırasında ortaya çıkabilmektedir. Biz bu çalışmada, eritrodermi tanısı ile takip edilen hastaların, klinik ve laboratuar özeliklerini araştırmayı amaçladık. Materyal-Metod: Kliniğimizde eritrodermi tanısıyla izlenen hastaların dosyalan, klinik ve laboratuar özellikleri, etyolojide rol oynayan faktörler ve hastalıklarının seyri açısından retrospektif olarak incelendi. Çalışmaya alınan 45 hastanın ortalama yaşlan 44.227.3 ve erkek/kadın oranı 3:2 idi. Bulgular: Daha önce var olan bir dermatolojik hastalığın alevlenmesi (% 53.3) en sık rastlanan etyolojik faktördü. On hastada (% 22.2) eritroderminin farklı gruplardan ilaç kullanımına bağlı olarak ortaya çıktığı saptandı. Dokuz hasta (% 20), etyolojide hiçbir faktör saptanamadığı ve histopatolojik olarak ayırt edilemediği için idiopatik eritrodermi olarak kabul edildi. İzlem sırasında 4 hasta sepsis komplikasyonu ile kaybedildi. Sonuç: Sonuç olarak, eritroderminin daha çok altta yatan dermatolojik hastalığın alevlenmesine bağlı olarak ortaya çıktığı ve ileri yaştaki erkek hastalarda daha sık olduğu gözlendi. Purpose: Erythroderma is characterized by erythema and desquamation of all or almost all skin and occurs during various dermatoses. In this study, we aimed to investigate the clinical and laboratory characteristics of erythrodermic patients, Material and Methods: Files of the patients diagnosed as erythroderma, were investigated for the clinical and laboratory features, etiologic factors and prognosis, retrospectively. The mean age of the 45 patients was 44.2±27.3, and male/female ratio was 3:2. Results: The most common etiological factor was the exacerbation of previously diagnosed dermatologie disorders (53.3 %). In 10 patients (22.2%) erythroderma was found to be caused by various drugs. Nine patients whose etiologic factors have not been identified, and histopathologic examinations were indifferent, were accepted as idiopathic erythroderma. Four patients died due to sepsis during hospitalization period. Conclusion: In conclusion, erythroderma was observed most commonly due to exacerbation of underlying dermatologie disorders in elderly men.

Objective: The sharp object injuries constitute an important problem for the healthcare staff. The aim of this study is to retrospectively evaluate percutaneous injuries in Inönü University, Faculty of Medicine in the last eight years. Materials and Methods: Percutaneous injuries that occurred in our hospital between 2006 and 2013 were evaluated respectively.. Results: 144 percutaneous injuries recorded during the last eight years were included in the study. The rates of exposure were found to be 44% (63) for cleaning personnel, 17% (25) for nurses, 12% (17) for medical students, 12% (17) ancillary staff and caregivers, 6% (9) for apprentice nurses, 5% (7) for technicians, and 4% (6) for doctors. The number of percutaneous injuries were found to be 126 (87%) for needle stick injuries, 11 (8%) for lancet injuries and 7 (5%) for mucosal (eye) contacts. We have observed a significant decrease in the total injuries and injured cleaning personnel, ancillary staff, and nurses (p<0.001). Conclusions: Throughout 8 years, percutaneous injuries were significantly reduced among healthcare staff. We plan to provide continuous education for healthcare personnel and follow up feedback in our hospital. [ABSTRACT FROM AUTHOR]

The title molecules,N-(1,5-dimethyl-3-oxo-2-phenyl-1H-3-(2H)-pyrazolyl)4-nitrobenzamide (C18H16N4O4·H2O) (I) and 2,2-dimethyl-3-(4-nitrobenzoyl)-5-(phenylamino)-2,3-dihydro-1,3,4-thiyadiazole (C17H16N4O3S) (II), were prepared and characterised by1H NMR,13C NMR, infrared spectroscopy (IR) and structural X-ray diffraction (XRD) techniques. The molecular geometries, vibrational frequencies of the title compounds in the ground state have been calculated by using the density functional theory (DFT) method with 6-31G(d) basis set, and compared with the experimental data. The calculated results showed that the optimised geometries from the DFT method agree with the X-ray structures well for both compounds. Theoretical calculations of harmonic vibration frequencies are in good agreement with experimental results. To determine conformational flexibility, the molecular energy profiles of the title compounds were obtained. Besides, molecular electrostatic potential (MEP), frontier molecular orbitals (FMO) analysis and thermodynamic properties of the title compounds were investigated by theoretical calculations. 178. The expanded spectrum of arthritis in children with familial Mediterranean fever.

Introduction: Familial Mediterranean fever (FMF) is the most common monogenic autoinflammatory disease that can present with various forms of arthritis. This retrospective study aims to evaluate the characteristics of patients with arthritis in a large pediatric cohort of FMF patients. Methods: The demographic and clinical data were extracted from electronic medical records. Patients with arthritis were grouped as arthritis of FMF and arthritis of FMF-associated diseases. Results: A total of 541 patients were followed with a diagnosis of FMF in the last 5 years. Acute arthritis of FMF (n: 138) was the most common cause. It showed a recurrent course in the majority with a longer duration than other attack symptoms. Significantly higher frequencies of biallelic exon 10 and M694V mutations, erysipelas-like erythema, and protracted febrile myalgia were detected in these patients, particularly in those older than 2 years of age. Sacroiliitis of FMF was the second most common cause (n: 19). Patients with acute arthritis and sacroiliitis of FMF needed higher doses of colchicine. One patient with neonatal-onset FMF and M694V homozygosity was diagnosed with protracted arthritis. Arthritides of FMF-associated diseases including IgA vasculitis (n: 10), juvenile idiopathic arthritis (n: 9), chronic nonbacterial osteomyelitis (n: 5), and inflammatory bowel disease (n: 2) were detected in 26 patients. Conclusions: Arthritis is an important clinical finding of FMF mostly associated with M694V mutations. The frequency of protracted arthritis is declined, whereas sacroiliitis of FMF and arthritis of associated diseases expand the spectrum of arthritis. This study represents the changing face and current perspectives of arthritis in FMF. Background and Design: In view of the evolving knowledge about the presence of the potent proinflammatory galectin-3, a member of the galectin family, in the cells (mainly T-lymphocytes) playing a key role in the inflammation process. We aimed to determine the serum galectin-3 levels in patients with psoriasis vulgaris, a T-cell-mediated inflammatory disease, and to investigate the correlation between these levels and disease activity. Material and Method: Thirty-three patients with psoriasis vulgaris and 30 healthy subjects were included in this study. The psoriasis area and severity index (PASI) was calculated and recorded for each patient. The patients were classified in three groups as group I, II and III according to their PASI scores. Serum galectin-3 level was analyzed by ELISA method. Results: Serum galectin-3 level in psoriasis vulgaris patients was found to be minimally, although statistically significantly, lower than the control group (p=0.012). We could not detect any correlation between serum galectin-3 level and PASI scores (Spearman's rho=0.206, p=0.249). Conclusion: It seems that serum galectin-3 level is not an important factor in the ethiopathogenesis of psoriasis vulgaris and, therefore, cannot be used as a reliable indicator of exacerbation during follow-up. A new dithiocarbamate molecule, named as N-benzoyl-S-(undecyl)-dithiocarbamate (C 19 H 29 NOS 2), was synthesized and characterized by 1HNMR, 13CNMR, FT-IR spectroscopic methods. X-ray analysis of the crystal structure of title compound showed the presence of triclinic space group with a = 4.2417 (8) Å, b = 20.010 (4) Å, c = 27.959 (6) Å, Z = 4, V = 2338.9 ų. Detailed investigation of molecular packing of the molecule indicated the presence of intermolecular hydrogen bond between C1–H1⋯S4 i and C24–H24⋯S2 v that generates R 2 2 (10) motifs, and intermolecular hydrogen bonds between N1–H111⋯S4 ii and N2–H222⋯S2 iv atoms that forms R 2 2 (7) rings. Thermal properties of the title compound were investigated by thermogravimetric analysis (DTA/TG) and differential scanning calorimetry (DSC). Molecular electrostatic potential (MEP), the HOMO and LUMO energies and thermodynamic parameters of the title compound were calculated using density functional theory (DFT) with B3LYP/6-311G (d,p) level. Letter to the editor regarding article "El-Hamd MA, El Taieb MA, Ibrahim HM, Aly SS. Vitamin D levels in acne vulgaris patients treated with oral isotretinoin. We read that serum levels of 25-hydroxy (OH) vitamin D were significantly lower in patients with acne vulgaris than healthy controls, and there was a significant increase in serum 25-OH vitamin D levels after 3 months of isotretinoin treatment. They also express nuclear vitamin D receptor which mediates the genomic actions of 1,25 (OH) SB 2 sb vitamin D.[[1]] So while evaluating the acne and vitamin D, two forms should be evaluated separately. 184. YOĞUN BAKİM ÜNİTESİNDE ÜRİK ASİT YÜKSEKLİĞİNİN AKUT BÖBREK HASARİ VE MORTALİTE ÜZERİNE ETKİSİ.

- Abstract
Amaç: Ürik asit pürin metabolizmasının son ürünüdür. Atılımının yaklaşık %80'i böbrekler yoluyla olmaktadır. Ürik asit yüksekliği; distal tubül ve toplayıcı kanallarda çökme sonucu akut ürik asit nefropatisine, medullanın interstisyumunda birikimiyle kronik ürat nefropatisine ve son olarak ürik asit taşları oluşturarak böbrek hasarına neden olabilir. Yoğun bakım ünitesine yatırılan hastalarda Akut Böbrek Hasarı (ABH) gelişiminin mortaliteyi arttırdığı bilinmektedir. Bizde bu bilgiler ışığında yoğun bakım ünitesine yatırılan hastalarda ürik asit yüksekliğinin akut böbrek hasarı gelişimi ve mortalite üzerine etkisini araştırmayı amaçladık. Gereç-Yöntem: Çalışmamızda 01 Ocak-01 Eylül 2019 tarihleri arasında Dicle Üniversitesi Tıp Fakültesi Genel Dahiliye Yoğun Bakım Ünitesine yatırılan cinsiyet ayrımı yapılmayan 110 hastanın verileri retrospektif olarak incelendi. Hastaların yaş, cinsiyet, başvuru anındaki serum ürik asit değerleri, serum kreatinin düzeylerine ve yoğun bakımda kalış sürelerine bakıldı. Akut Böbrek Hasarı KDIGO kriterleri kullanılarak değerlendirildi. Hastalar ürik asit değeri <7.2 mg/dl (Grup 1) ve >=7.2 mg /dl (Grup 2) olacak şekilde iki gruba ayrıldı (Hastanemiz serum ürik asit referans değerleri 2.6-7.2 mg/dl). Gruplar ABH gelişimi ve mortalite açısından karşılaştırıldı. Bulgular: Hastaların 54'ü erkek, 56'sı kadın hastaydı. 74 hastada (%67.3) ABH gelişti ve 70 hasta (%63.6) ex oldu. Grup 1'de 51 hasta, Grup 2 'de 59 hasta mevcuttu. Gruplar arasında cinsiyet açısından istatiksel olarak anlamlı fark saptanmadı (p:0.257). Grup 1 ve Grup 2'de ABH gelişme oranı sırasıyla %35.3- %94.9 saptanırken ex olanların oranı %45- %79.6 olup bu bulgular istatiksel olarak anlamlıydı (p<0.01). Gruplar arası dağılım Tablo 1'de gösterilmiştir. Yoğun bakım ünitesinde kalış süreleri median değer olarak gruplar arasında sırasıyla 13 (%95 CI: 8.9-17.2) ve 6 (%95 CI: 4.2-7.8) olup Log rank testiyle grup 2 de istatiksel olarak anlamlı düşük saptandı (p:0.003). Grupların Kaplan-meier sağ kalım analizi şekil 1'de gösterilmiştir. Cox regresyon analiziyle serum ürik asit değerinin >=7.2 mg /dl olması ABH'ını 2.9 kat (p<0.001) ve mortaliteyi 2.1 kat (p:0.005) arttırdığını tespit ettik. Sonuç: Çalışmamızda serum ürik asit yüksekliğinin ABH'ı ve mortaliteyi arttırdığı görülmektedir. Bu nedenle yoğun bakım hastalarında serum ürik asit düzeyinin yakın takip edilmesi gerekmektedir. Hiperürisemi saptanan hastalarda uygun medikal tedavi ile serum ürik asit düzeyinin düşürülmesi hem ABH'nı hem de mortaliteyi azaltabileceği kanaatindeyiz. [ABSTRACT FROM AUTHOR]

- Abstract
Objective: In this study, it was aimed to determine the HCV prevalence and the occult HBV prevalence by HBV DNA analysis in serum samples of HBsAg patients undergoing routine hemodialysis. It was also aimed to determine the liver fibrosis level with fibrometer, which is a non-invasive method. Methods: The study included 100 patients who applied for routine hemodialysis and were negative for HBsAg in screening tests. In samples from the patient, ALT, HBsAg, Anti-HBc IgG, HBV DNA, Anti HCV, HCV RNA and fibrometer were studied from serum samples from the same system. HBsAg negative and HBV DNA positive occult hepatitis B were accepted. Results: HBV DNA was found positive in 100 positive 4 (4%). Viral load was <10 IU/ml in two of the HBV DNA positive cases, and 17 and 16 IU/ml in the other two cases. While Anti HCV positive was detected in one patient; HCV RNA could not be detected in any patient. While there was a significant difference in scores compared to those who did not have occult B; The score of histological activity and necrosis were not significantly different. Conclusion: Since antigen asntibody-based serology tests may be insufficient in the diagnosis of occult HBV, determination of HBV DNA level in special groups such as hemodialysis patients is important in terms of early recognition of occult HBV and preventing important complications such as cirrhosis and hepatocellular cancer. The absence of a previous study on occult hepatitis B and fibrometer in the literature has increased the importance of this study. [ABSTRACT FROM AUTHOR]

The SAPHO syndrome was given its acronymic name based on the presence of Synovitis, Acne, Pustulosis, Hyperostosis and Osteitis in patients. The presence of joint and bone pain in patients with treatment resistance severe acne should suggest SAPHO syndrome. In addition, isotretinoin is an appropriate treatment for severe, scarring acne and may also improve osteoarticular manifestations.2,3,8 The current study described a rare case of SAPHO syndrome with diffuse nodulocystic acne, and musculoskeletal involvement of the sacroiliac joint, vertebrae and anterior chest wall arthritis or osteitis. 187. Maternal and fetal Doppler velocimetry in women diagnosed with fear of childbirth.

Objective: We hypothesised that women with fear of childbirth (FOC) may have increased vascular resistance in maternal and foetal vascular beds due to maternal psychological stress. For this purpose, we investigated pulsatility (PI) and resistance (RI) index values for uterine, umbilical and mid cerebral arter ies in term pregnant women who were diagnosed with FOC and we compared these values than those of women without FOC. Methods: Women between 20-40 years with full-term singleton pregnancies (⩾37 gestational weeks) were included. All patients were questioned with Turkish form of Wijma Delivery Expectancy/Experience Questionnaire (W-DEQ) version A. Maternal age, gestational week, parity, body mass index and total W-DEQ scores of patients were recorded. Women with W-DEQ scores ⩾85 was defined as FOC. Forty women diagnosed with FOC (FOC group) and 45 women with W-DEQ scores <85 (control group) underwent Doppler waveform analysis and the pulsatility (PI) and resistance (RI) index values for uterine, umbilical and mid cerebral arteries were recorded. Data were analysed using SPSS software 12.0 (SPSS Inc., Chicago, IL, USA) and expressed as mean ⩾ standard deviation, P < 0.05 was considered significant. The normality of variables was tested using the Kolmogorov-Smirnov test. If the data was not normally distributed, Mann-Whitney U-test was used. Comparisons were done using the independent samples t-test when the data was normally distributed and the Fisher's exact test was used to compare the percentage values. Results: There were no differences between groups in terms of maternal age, parity, body mass index and gestational week values. Control group had lower W-DEQ scores compared with FOC group (p<0.001). Both groups had similar PI and RI values for umbilical and mid cerebral arteries (p>0.05). However, PI and RI values for both right and left uterine arteries were higher in FOC group than control group (p<0.05, for right uterine artery PI; p< 0.001, for left uterine artery RI and p<0.01, for others). Conclusions: It may be suggested that the presence of FOC in term pregnant women seems to have a negative effect on uterine blood flow parameters. When diagnosed with FOC, the women should be referred to a specialist for psycho-education and psychosomatic support in order to decrease her fear and to minimize the negative impact of fear on the foetus. Autoinflammatory diseases (AIDs) are a recently described group of conditions caused by mutations in multiple genes that code for proteins of the innate immune system. Cryopyrin-associated periodic syndromes (CAPS) are autoinflammatory diseases comprising three clinically overlapping disorders: familial cold urticarial syndrome (FCAS), Muckle-Wells syndrome (MWS), and neonatal-onset multisystem inflammatory disease (NOMID). CAPS have been associated with gain-of-function variations in NLRP3 (NOD-like receptor family, pyrin containing domain-3). However, a new class of autoinflammatory disease resembling FCAS or MWS has been described in patients with NLRP12 mutations. Here, we report a 6-year-old boy diagnosed with AID who developed an unexpected C3 glomerulopathy during attacks and carried a novel variation in NLRP12. Following treatment with IL (interleukin) 1 targeting agents, all symptoms and inflammation resolved. This is the first case in the literature affected by both autoinflammatory disease and C3 glomerulopathy. [ABSTRACT FROM AUTHOR]

Two new dithiocarbamate molecules were synthesized and characterized by 1 H NMR, 13 C NMR, IR, and structural X-ray diffraction techniques. The molecular geometry, vibrational frequencies of the title compounds in the ground state have been calculated by using the Hartree-Fock (HF) and density functional theory (DFT) methods with 6–31G(d) basis set, also gauge-independent atomic orbital (GIAO) 1 H and 13 C NMR chemical shift values of the title compound (I) have been calculated by the same methods and compared with the experimental data. The calculated results show that the optimized geometries can well reproduce the crystal structural parameters. A detailed vibrational spectral analysis has been carried out and assignments of observed fundamental bands have been proposed on basis of peak positions. The scaled theoretical frequencies showed very good agreement with experimental values. In addition, the computed 1 H and 13 C NMR chemical shift values are in line with experimental data. To determine conformational flexibility, the molecular energy profile of the title compounds were obtained in respect of the selected torsion angle, which were varied from −180° to +180° in steps of 10°. Besides, frontier molecular orbitals (FMO), molecular electrostatic potential (MEP)analysis and thermodynamic properties were investigated by theoretical calculations. [ABSTRACT FROM AUTHOR]

Background: The purpose of this study was to compare the demographic, clinical and laboratory characteristics of patients with enthesitis‐related arthritis (ERA), familial Mediterranean fever (FMF) and inflammatory bowel disease (IBD), which are inflammatory diseases that may develop sacroiliitis. Thus, it was aimed to reveal various findings that may indicate primary disease in patients with sacroiliitis. Methods: Pediatric patients aged 6–18 years, who were being followed with a diagnosis of ERA (n = 62), FMF (n = 590), and IBD (n = 56) over the period 2013–2021 were included in the study. Sacroiliitis (n = 55) was diagnosed by magnetic resonance imaging of the sacroiliac joint, obtained from clinically suspected patients. Results: Sacroiliitis was detected in 54.8% of ERA patients, 2.3% of FMF patients, and 12.5% of IBD patients. The mean follow‐up period was 4.1 ± 2.8 years (10 months‐8 years) for the entire study group. The most common MRI finding for sacroiliitis was bone marrow edema. Peripheral joint involvement (73.5%) and HLA B27 positivity (64.7%) was significantly higher in ERA patients, and ERA was diagnosed more frequently in patients presenting with sacroiliitis. Non‐steroidal anti‐inflammatory drugs (NSAIDs) were the first choice of treatment agent when sacroiliitis developed in all three patient groups. Conclusions: The clinical and laboratory findings of ERA, FMF and IBD can sometimes be intertwined or can even coexist. Treatment may differ depending on the disease associated with sacroiliitis, although NSAIDs may be used in the first‐line treatment of all three diseases. Sacroiliitis patients with HLA B27 positivity and peripheral arthritis may need to be addressed as ERA. [ABSTRACT FROM AUTHOR]

Objective: In this study, it was aimed to evaluate the demographic, clinical and laboratory characteristics of MIS-C patients in our hospital, to share our treatment approach, and to assess the outcomes of short- and long-term follow-up. Methods: MIS-C patients who were admitted and treated in our hospital between July 2020 and July 2021 were evaluated. Demographic, clinical, laboratory, and follow-up data were collected from patient records retrospectively. Results: A total of 123 patients with MIS-C (median age, 9.6 years) were included the study. Nineteen (15.4%) were mild, 56 (45.6%) were moderate, and 48 (39%) were severe MIS-C. High CRP, ferritin, pro-BNP, troponin, IL-6, and D-dimer values were found in proportion to the severity of the disease (p < 0.001, p < 0.001, p < 0.001, p < 0.001, p = 0.005, p < 0.001), respectively. Two (1.6%) patients died. The mean follow-up period was 7.8 months. Valve failure, left ventricular dysfunction/hypertrophy, coronary involvement, and pericardial effusion were the most common cardiac pathologies in the short- and long-term follow-up of the patients. In the long-term follow-up, the most common reasons for admission to the hospital were recurrent abdominal pain (14.2%), cardiac findings (14.2%), pulmonary symptoms (8%), fever (7.1%), neuropsychiatric findings (6.2%) and hypertension (3.5%). Neuropsychiatric abnormalities were observed significantly more common in severe MIS-C patients at follow-up (p = 0.016). In the follow-up, 6.2% of the patients required recurrent hospitalization. Conclusion: MIS-C is a serious and life-threatening disease, according to short-term outcomes. In addition to the cardiac findings of patients with MIS-C, long-term outcomes such as neuropsychiatric findings, persistent gastrointestinal symptoms, fever and pulmonary symptoms should be monitored. Key Points • In MIS-C patients, attention should be paid not only to cardiac findings, but also to symptoms related to other systems. • Patients should be followed up in terms of neuropsychiatric findings, persistent gastrointestinal symptoms, fever and pulmonary symptoms that may occur during follow-up. Multisystem inflammatory syndrome in children (MIS-C) is a life-threatening hyperinflammation syndrome emerging after COVID-19. The serum delta neutrophil index (DNI) reflects the fraction of circulating immature granulocytes and is evaluated in infection and inflammation. The aim of this study is to evaluate the usefulness of DNI as a diagnostic marker in patients with MIS-C and to assess its role in determining the severity of MIS-C. This retrospective, observational study included 83 patients with MIS-C and 113 patients with COVID-19, and 102 healthy controls. C-reactive protein (CRP), the absolute neutrophil count (ANC), absolute lymphocyte count (ALC), DNI, and the platelet count were recorded. The DNI levels were 4.60 ± 5.70% in the MIS C group, 0.30 ± 0.99% in the COVID group, and 0.20 ± 0.56% in the control group (p < 0.001). According to the severity of MIS-C, the DNI level was found to be 1.22% in mild MIS-C, 4.3% in moderate MIS-C, and 5.7% in severe MIS-C. There was a statistically significant correlation between DNI levels and the severity of MIS-C. The cutoff value of DNI for predicting MIS-C was 0.45%. In the analysis of the diagnostic performance of DNI compared with CRP, ANC, ALC and platelet counts, sensitivity, specificity, positive predictive value, and negative predictive value were found to be 79.5%, 97.1%, 95.7%, and 85.3%, respectively. Conclusions: The delta neutrophil index was identified as a diagnostic marker for MIS-C such as ANC, ALC, platelet count, and CRP. DNI levels in hemogram analysis may guide clinicians in determining the diagnosis and severity of MIS-C. What is Known: • Although CRP, sedimentation, ALC, ANC, platelet count, sodium, and albumin are used as first step tests, there is no specific laboratory marker used in the diagnosis of MIS C. • The serum delta neutrophil index (DNI) reflects the fraction of circulating immature granulocytes and is elevated in infection and inflammation. What is New: • DNI is a promising and easily accessible marker that can be used with other markers in the diagnosis and determines the severity of MIS C. • DNI is an easily accessible, inexpensive, and dynamic marker and its levels in simple hemogram analysis will guide pediatricians in determining the diagnosis and severity in MIS C. Abstract: The title molecule (C19H17N5O4S·H2O) was synthesized and characterized by IR-NMR spectroscopy, MS and single-crystal X-ray diffraction. The molecular geometry, vibrational frequencies and gauge-independent atomic orbital (GIAO) 1H and 13C NMR chemical shift values of the compound in the ground state have been calculated by using the density functional theory (DFT) method with 6-31G(d) basis set, and compared with the experimental data. All the assignments of the theoretical frequencies were performed by potential energy distributions using VEDA 4 program. The calculated results show that the optimized geometries can well reproduce the crystal structural parameters, and the theoretical vibrational frequencies and 1H and 13C NMR chemical shift values show good agreement with experimental data. To determine conformational flexibility, the molecular energy profile of the title compound was obtained with respect to the selected torsion angle, which was varied from −180° to +180° in steps of 10°. Besides, molecular electrostatic potential (MEP), frontier molecular orbitals (FMO) analysis and thermodynamic properties of the compound were investigated by theoretical calculations. Background: A good glycemic regulation should be provided to reduce mortality and morbidity in patients with end-stage renal failure due to diabetes mellitus. However, the use of insulin therapies in patients with renal failure is troublesome due to the increased rate of side effects. In our study, we investigated the frequency of hypoglycemia and its symptoms in patient groups receiving intensive and mixed insulin therapies. Materials and methods: This prospective study included 89 hemodialysis patients with DM-related stage 5 chronic kidney disease who were followed up in a nephrology clinic of a university hospital between January 2013 and August 2015. Our study group was divided into two groups as those receiving intensive insulin therapy and mixed insulin therapy. Group 1 and group 2 consisted of 46 patients and 43 patients, respectively. Hypoglycemia (glucose < 70 mg/dl) was investigated in patients with HbA1c levels that decreased below 7% after treatment, and the symptoms were evaluated according to the hypoglycemic scale. The results between the groups were evaluated using chi-square and Student's t test Results: A total of 89 patients were included in the study. HbA1c was 6.83% in the group receiving intensive insulin therapy and 6.95% in the group receiving mixed insulin therapy (p > 0.05). Hypoglycemia was detected in 27 patients (58.7%) in the intensive insulin therapy group and 14 patients (32.6%) in the mixed insulin therapy group. There was a significant level of hypoglycemia in the group receiving intensive insulin therapy (p < 0.05). In our study, the group receiving intensive insulin therapy had higher frequency and severity of common hypoglycemia symptoms such as confusion, sweating, weakness, dysphasia, palpitations, blurred vision, and feeling hungry. Conclusion: There was a higher frequency of hypoglycemia under intensive insulin therapy in patients undergoing dialysis due to chronic kidney failure, which suggests that mixed insulin therapy is the ideal treatment to avoid hypoglycemia in this group of patients. WOS:000758928100011 PMID: 35242474 Abstract Introduction and aim Acute kidney injury (AKI) is part of the multiple organ dysfunction syndrome in critically ill patients and is a common condition in intensive care units (ICUs). Renal replacement therapy (RRT) is the cornerstone of treatment for AKI in critically ill patients. This patient population has a high mortality rate despite RRT. There are two methods of RRT for patients in ICUs: intermittent hemodialysis (IHD) and continuous renal replacement therapy (CRRT). Both CRRT and IHD similarly provide adequate metabolic control. We aimed to compare these two RRT modalities in terms of ICU stay, mortality, and laboratory recovery in these patients with high mortality. Materials and methods A total of 120 patients with AKI who needed RRT in the ICU were included in the study (CRRT, n:40; IHD, n:80). Acute Physiology and Chronic Health Evaluation (APACHE) II, Sepsis-related Organ Failure Assessment (SOFA), and Simplified Acute Physiology Score (SAPS)-II scores at the time of admission to the ICU were calculated. Mean arterial pressure, urea, creatinine, sodium, potassium, calcium, pH, lactate, and bicarbonate levels were measured before and after dialysis. Patients were classified as living and deceased. Factors affecting the length of stay in the intensive care unit and 30-day mortality were evaluated. The variability in laboratory parameters between groups before and after dialysis was examined. The groups were compared with these parameters. Results Sixty-one point seven percent (61.7%, n:74) of the patients were female. The mean age was 62.90 +/- 13.64 years. At the time of admission to the ICU, the patients' SAPS II score was 45.05 +/- 12.76, APACHE II score was 22.05 +/- 6.32, and SOFA score was 8.26 +/- 2.48. 66.7% (n:80) of the patients included in the study died, and the length of stay of these patients in the ICU was 12.85 +/- 10.23 days. When the groups were compared, SAPS II, APACHE II scores, and SOFA scores were significantly higher in the CRRT group than in the IHD group (p:0.038, p:0.015, p:0.027, respectively). Although the length of stay in the ICU was shorter in the CRRT group, it was not statistically significant (p:0.075). There was no statistically significant difference between the groups in terms of mortality (p: 0.891). SAPS-II, APACHE II, and SOFA score affected 30-day mortality while age, gender, and RRT modalities were not associated with mortality. Background: reduced memory specificity (i.e., overgeneral memory) is a characteristic of autobiographical memories widely studied in clinical populations, and it is explained by rumination, functional avoidance, and executive dysfunction. Though the relationship of autobiographical memory specificity with mood and anxiety disorders has been shown, how it relates to dissociation is not well-established. Thus, we aimed to investigate whether dissociative experiences are related to overgeneral memory while considering concurrent depression as a possible confounding factor. Methods: we conducted a systematic review in compliance with The Preferred Reporting Items for Systematic reviews and Meta-Analyses (PRISMA) guidelines and searched PubMed and Web of Science databases using autobiograph* and dissoc* as our keywords. Results: of the 768 studies identified, 9 studies fulfilled the inclusion criteria. A meta-regression analysis was conducted to analyze the relationship between dissociative experiences and depression scores with autobiographical memory test scores. Our research revealed that depression scores, but not dissociative experiences, are significantly related to reduced memory specificity. Conclusion: while the possible overlap between dissociation and depression should be considered in the interpretation of the findings, dissociative experiences do not seem to pose vulnerability for reduced specificity of autobiographical memory. The number of studies on the topic is limited, and they do not have longitudinal follow-ups. Introduction: Hospital-acquired infections (HAI) occurring in intensive care units (ICUs) are an important risk factor for mortality and morbidity. In some patient groups followed in ICUs, the risk of developing nosocomial infections increases even more. Especially, patients with end-stage renal disease (ESRD) carry a serious risk for HAI when they are hospitalized in ICUs. Our aim is to determine the rate and incidence of hospital infection, the distribution of infections and the most common microorganisms in our ICU, to initiate appropriate empirical treatment and to prevent the development of antibiotic resistance. Material and Method: A total 158 patients with a diagnosis of hospital-acquired infection hospitalized between January 2017 and December 2019 at general internal medicine intensive care unit, were included in this study. The clinical findings, culture results and laboratory data of the patients were recorded. According to years, the HAI rate, density and infection agents in the ICU were determined. Results: 158 episodes of nosocomial infections were detected in 128 of 556 patients who were hospitalized within three years. The hospitalization day was 9048, and the three-year ICU HAI rate was calculated as 29.19%. HAI density was 17.45 in 1000 patient days. Bloodstream infection was the most common (30.38%), followed by ventilator-associated pneumonia (28.48%) and catheter-related urinary tract infection (24.68%). Gram-negative microorganisms were the most common infectious agents. Among the Gram-negative bacteria, the most frequently isolated bacteria were A. baumannii, K. pneumoniae and P. aeruginosa. Among Gram-positive bacteria, Enterococcus spp. was most frequently isolated Conclusion: The risk of HAI is high in patients hospitalized in ICUs. In order to control nosocomial infections, HAI incidences and rates should be evaluated, infectious agents, and prospective effective infection control strategies should be developed by taking necessary precautions according to surveillance results. These measures will significantly reduce the incidence of HAI.

Introduction: Monocyte to high-density lipoprotein (HDL) cholesterol ratio (MHR) has emerged as a novel marker of endothelial injury, inflammation, and oxidative stress. This study aimed to investigate the effect of MHR on primary nephrotic syndrome (NS) and its relationship with the severity of proteinuria. Methods: This study enrolled 161 patients newly diagnosed with primary NS and 100 healthy individuals. Demographic characteristics of the patients, primary NS diagnosis, basal laboratory parameters, the amount of 24-hour urinary protein excretion, and MHR were recorded. The groups were compared regarding these parameters. Results: MHR was significantly higher in patients with primary NS compared with the healthy group (12.89 +/- 4.86 and 9.71 +/- 2.30, respectively; p < 0.001). There was no difference between the groups in terms of age and sex. The amount of protein in the 24-hour urine in patients with a diagnosis of primary NS was 6.91 +/- 3.73 g/day. The correlation analysis showed a positive correlation between MHR and the amount of proteinuria (r = 0.519, p < 0.001) and creatinine level (r = 0.167, p = 0.034). The multivariate regression analysis found that the severity of proteinuria was independently correlated to MHR (p < 0.001). According to the receiver operating characteristic curve analysis, the optimal cut-off level for MHR in NS was 10.08 (area under the curve of 0.704, sensitivity of 68%, and a specificity of 62%). Conclusion: Our study is the first to compare the severity of proteinuria and renal functions with MHR in patients with primary NS. We believe that MHR can be used as a biomarker to determine inflammation, endothelial injury, and the level of oxidative stress, and may be useful to predict prognosis in patients with primary NS.
