- Abstract
Context: Abciximab infusion and uncoated-stent implantation is a complementary treatment strategy to reduce major adverse cardiac events in patients undergoing angioplasty for ST-segment elevation myocardial infarction (STEMI). It is uncertain whether there may be similar benefits in replacing abciximab with high-dose bolus tirofiban. Similarly, the use of drug-eluting stents in this patient population is currently discouraged because of conflicting results on efficacy reported in randomized trials and safety concerns reported by registries.Objective: To evaluate the effect of high-dose bolus tirofiban and of sirolimus-eluting stents as compared with abciximab infusion and uncoated-stent implantation in patients with STEMI undergoing percutaneous coronary intervention.Design, Setting, and Patients: An open-label, 2 x 2 factorial trial of 745 patients presenting with STEMI or new left bundle-branch block at 16 referral centers in Italy, Spain, and Argentina between October 2004 and April 2007.Interventions: High-dose bolus tirofiban vs abciximab infusion and sirolimus-eluting stent vs uncoated stent implantation.Main Outcome Measures: For drug comparison, at least 50% ST-segment elevation resolution at 90 minutes postintervention with a prespecified noninferiority margin of 9% difference (relative risk, 0.89); for stent comparison, the rate of major adverse cardiac events, defined as the composite of death from any cause, reinfarction, and clinically driven target-vessel revascularization within 8 months.Results: ST-segment resolution occurred in 302 of 361 patients (83.6%) who had received abciximab infusion and 308 of 361 (85.3%) who had received tirofiban infusion (relative risk, 1.020; 97.5% confidence interval, 0.958-1.086; P < .001 for noninferiority). Ischemic and hemorrhagic outcomes were similar in the tirofiban and abciximab groups. At 8 months, major adverse cardiac events occurred in 54 patients (14.5%) with uncoated stents and 29 (7.8%) with sirolimus stents (P = .004), predominantly reflecting a reduction of revascularization rates (10.2% vs 3.2%). The incidence of stent thrombosis was similar in the 2 stent groups.Conclusions: In patients with STEMI undergoing percutaneous coronary intervention, compared with abciximab, tirofiban therapy was associated with noninferior resolution of ST-segment elevation at 90 minutes following coronary intervention, whereas sirolimus-eluting stent implantation was associated with a significantly lower risk of major adverse cardiac events than uncoated stents within 8 months after intervention.Trial Registration: clinicaltrials.gov Identifier: NCT00229515. 7 Be decays via electron capture and therefore its half-life is expected to depend on the electron density at the nucleus. We measured the7 Be half-life in palladium, tungsten, zirconium and tantalum metals, in order to investigate the influence of the quasi-free electrons in metals on the probability of electron capture. The7 Be samples were obtained implanting a 4.85MeV pure radioactive7 Be ion beam. The7 Be half-life was determined measuring the 478keV gamma decay following the electron capture by means of HPGe detectors. In order to reduce systematic errors, we planned to perform independent measurements in three different laboratories: Naples (Italy), Bochum (Germany) and Debrecen (Hungary). 109. Afterload reduction: a comparison of captopril and nifedipine in dilated cardiomyopathy.

- Abstract
Nifedipine and captopril are potent vasodilators and may be expected to help left ventricular failure by reducing afterload. Nifedipine (20 mg three times a day) and captopril (50 mg three times a day) were added to an optimal regimen of digitalis and diuretics in a double blind crossover trial in 18 cases of dilated cardiomyopathy. New York Heart Association functional class rating symptoms and exercise tolerance times improved on captopril but not on nifedipine. The reduction in pulmonary capillary wedge pressure and the increase of cardiac output on captopril indicated that the augmented functional capacity may have resulted in part from an improved performance of the left ventricle. Although there were comparable decreases in systemic vascular resistance and presumably in impedence to ejection by the left ventricle on both drugs, the dimensions of the ventricular cavity were found to be reduced by captopril and augmented by nifedipine, and only captopril reduced the afterload (wall stress). In addition, the force-length relation (between left ventricular end systolic stress and end systolic diameter) was shifted to the left of baseline by captopril and to the right by nifedipine, suggesting that muscle contractility was reduced by nifedipine and not by captopril. These results suggest that nifedipine and captopril have different effects on afterload and contractility and these may account for the different effects of these drugs on the performance of the heart and clinical responses. In 24 patients with spontaneous and effort-related angina (mixed angina), propranolol (80 mg q.i.d.) was significantly more beneficial than nifedipine {20 mg q.i.d.) on the number, duration and severity of the spontaneous manifestations. In some cases nifedipine elicited a paradoxical response. These patterns are unlikely to have resulted from different influences on the myocardial oxygen demands, since heart rate was steady before the occurrence of ischaemia and systemic arterial pressure was equally reduced in all patients. Sublingual nifedipine (10 mg) was tested in 12 patients and the residual lumen diameter of significant (< 50%) coronary stenoses quantitative angiography) was unchanged in one, enhanced in seven and reduced in four of them. Lumen variations ranged from +1.59 to −1.2 mm and correlated closely with the results of oral nifedipine treatment. Propranolol (0.1 mg kg−1 i.v.) was tested in the other 12 cases and in none did variations of stenosis lumen diameter exceed 0.3 mm.These observations indicate that, in a number of lesions a portion of pliable wall may offer a compliant substrate for vasomotor influences; this may be the major factor whereby coronary obstructions cause spontaneous, besides effort-related angina; nifedipine is effective on the former manifestation provided that it does not promote stenosis constriction; propranolol may result in benefit through bradycardia facilitating coronary flow in diastole and reducing the baseline metabolic demands, to elevate the threshold of ischaemia during transient impedance to flow. 7Be(p,γ
)8B has an important role in nuclear astrophysics, having a direct impact on both the high energy component of solar neutrinos and the 7Li abundance after the Big Bang Nucleosynthesis. All direct measurements providing useful information on this reaction so far used the same approach, i.e. a proton beam on a radioactive 7Be target. The overall precision and accuracy of the estimate of the astrophysical rate of this reaction are limited by the discrepancy between the results of existing measurements, possibly due to the complicated stoichiometry and beam induced deterioration of the radioactive targets. The ERNA (European Recoil separator for Nuclear Astrophysics) collaboration planned a new experiment in inverse kinematics exploiting the intense 7Be beam available at CIRCE (Center for Isotopic Research on Cultural and Environmental heritage), Caserta, Italy. The 8B recoils are produced in a windowless hydrogen gas target and detected after the efficient mass separation provided by ERNA. Abstract: We report here on the direct generation of a CN− ion beam for the production of a N ion beam to be used in nuclear astrophysics measurements. The procedure relies on the production of CN− ion beam with a SNICS source starting from several appropriate substrates containing CN− triple bond. Several of the investigated substrates showed a higher beam intensity with respect to the common N beam production from a cathode of BN plus graphite. Best results in terms of analyzed beam intensity were observed with K3Fe(CN)6 and KSCN cathodes. Use of the latter compound is particularly appealing because of the easy availability of mass-15 enriched molecule. [Copyright &y& Elsevier]

The reaction 12C(α, γ)16O plays a key role in determining the C/O ratio in the stellar core at the end of helium burning, which in turn influences the further evolution and nucleosynthesis. The determination of the cross section at the relevant astrophysical energy , which is of the order of 10−17 b, requires extrapolation of the S factor determined from data collected with high precision and accuracy at higher energy. A new experimental approach has been undertaken in order to study this reaction for the first time by the direct detection of the 16O nuclides using the recoil mass separator ERNA. A 7Be ion beam of several particle pA at 8 MeV has been produced at the TTT3 tandem of the University “Federico II” in Naples. The 7Be nuclides were formed via the 7Li(p,n)7Be reaction using a metallic Li target and an 11.4 MeV proton beam of 20

For improved cross section measurements of the reaction 12 C ( α , γ ) 16 O in inverted kinematics, a recoil separator ERNA is developed at the 4 MV Dynamitron tandem accelerator in Bochum to detect directly the 16O recoils with high efficiency. The 16O recoils are produced by the 12C projectiles in a windowless 4He gas target. We report on the charge state distribution of the 16O recoils, the gas target density, the beam heating of the gas target, and the acceptance of the separator along the extended gas target.

Abstract: Accelerator mass spectrometry (AMS) is presently the most sensitive technique for the measurement of long-lived actinides, e.g. 236U and 239Pu. A new actinide line is in operation at the Center for Isotopic Research on Cultural and Environmental heritage (CIRCE) in Caserta, Italy. Using the actinide line a uranium mass sensitivity of around 4μg has been reached measuring with a 16-strip silicon detector, and a 239Pu background level of below 0.1fg has been obtained. In this work we also discuss preliminary results for environmental and structural samples from in and around the Garigliano nuclear power plant (GNPP), presently in the decommissioning phase. Measurements on environmental samples from the vicinity of the plant allow the assessment of contamination, if any, over the years. Measurements of structural samples from the plant are relevant to the optimization of the decommissioning program for the GNPP. [Copyright &y& Elsevier]

Abstract: The reaction plays a key role for the understanding of the production of 7Li in Big Bang nucleosynthesis as well as for the high energy component of the solar neutrino spectrum. The uncertainty in its absolute cross-section arises from systematic differences of the available data sets. In various experiments the cross-section was measured by the detection of the prompt -rays or by the off-line observation of the -rays following the 7Be electron capture; in a few cases both observations were carried out. We report on a new approach based on the use of the recoil separator ERNA (European Recoil separator for Nuclear Astrophysics) to detect directly the 7Be recoils as well as the prompt -rays in coincidence with the recoils. We present technical details of the experimental setup, such as a recirculating 3He gas target, a TOF-E end detector, and a NaI detector array. In addition, the measurement of the separator acceptance is discussed and compared to the expected recoil emittance. The electron capture half life of 7Be implanted into different metals has been measured with high precision in order to search for a possible dependence of the decay rate on the electron configuration around the 7Be atoms. In order to reduce systematic errors, the half life in different metals is measured by three laboratories simultaneously using different experimental setups. The measurements and data analysis are in progress, preliminary results are presented. [Copyright &y& Elsevier]

A reduction in mean LDL cholesterol to 1.4-1.8 mmol/L with ezetimibe or statins reduces cardiovascular events in patients with an acute coronary syndrome and diabetes. However, the efficacy and safety of further reduction in LDL cholesterol with an inhibitor of proprotein convertase subtilisin/kexin type 9 (PCSK9) after acute coronary syndrome is unknown. We aimed to explore this issue in a prespecified analysis of the ODYSSEY OUTCOMES trial of the PCSK9 inhibitor alirocumab, assessing its effects on cardiovascular outcomes by baseline glycaemic status, while also assessing its effects on glycaemic measures including risk of new-onset diabetes. Methods ODYSSEY OUTCOMES was a randomised, double-blind, placebo-controlled trial, done at 1315 sites in 57 countries, that compared alirocumab with placebo in patients who had been admitted to hospital with an acute coronary syndrome (myocardial infarction or unstable angina) 1-12 months before randomisation and who had raised concentrations of atherogenic lipoproteins despite use of high-intensity statins. Patients were randomly assigned (1: 1) to receive alirocumab or placebo every 2 weeks; randomisation was stratified by country and was done centrally with an interactive voice-response or web-response system. Alirocumab was titrated to target LDL cholesterol concentrations of 0.65-1.30 mmol/L. in this prespecified analysis, we investigated the effect of alirocumab on cardiovascular events by glycaemic status at baseline (diabetes, prediabetes, or normoglycaemia)-defined on the basis of patient history, review of medical records, or baseline HbA(1c) or fasting serum glucose-and risk of new-onset diabetes among those without diabetes at baseline. the primary endpoint was a composite of death from coronary heart disease, non-fatal myocardial infarction, fatal or non-fatal ischaemic stroke, or unstable angina requiring hospital admission. ODYSSEY OUTCOMES is registered with ClinicalTrials. gov, number NCT01663402. Findings At study baseline, 5444 patients (28.8%) had diabetes, 8246 (43.6%) had prediabetes, and 5234 (27.7%) had normoglycaemia. There were no significant differences across glycaemic categories in median LDL cholesterol at baseline (2.20-2.28 mmol/L), after 4 months' treatment with alirocumab (0.80 mmol/L), or after 4 months' treatment with placebo (2.25-2.28 mmol/L). in the placebo group, the incidence of the primary endpoint over a median of 2.8 years was greater in patients with diabetes (16.4%) than in those with prediabetes (9.2%) or normoglycaemia (8.5%); hazard ratio (HR) for diabetes versus normoglycaemia 2.09 (95% CI 1.78-2.46, p, Sanofi; Regeneron Pharmaceuticals, Sanofi and Regeneron Pharmaceuticals.

A pure 7 Be beam with an energy E = 1–8 MeV is available for nuclear and applied physics at the 3 MV Pelletron tandem accelerator CIRCE in Caserta. The beam is produced using an offline technique. Typical analyzed beam intensities are about 2 ppA, using cathodes with an activity of the order of 200 MBq. The 7 Be implantation has been used for both fundamental nuclear physics and applied physics. In particular, different metals have been implanted with 7 Be in order to study the influence of the chemical composition and of the number of quasi-free electrons of the host material on the 7 Be half-life. In the field of applied physics, the 7 Be implantation turns out to be very interesting for wear measurement. In fact, in this case 7 Be is used as a depth-sensitive tracer. The continuous detection of the sample activity during the wear allows a high sensitivity measurement of wearing speed. The 7 Be beam production at CIRCE, the implantation procedure and the results obtained from the 7 Be half-life measurements and the wear characterization of implanted steel samples are described.

The pretreatment of samples for radiocarbon measurements, transforming a variety of materials into graphite solid targets, represents a critical point in the accelerator mass spectrometry (AMS) procedure. We describe the new, state-of-the-art CIRCE AMS preparation laboratory, particularly the setup and optimization of an alternative method, the zinc reduction method, for graphite target production, compared to the more common hydrogen reduction method. Measured 14C values on standard and blank samples reduced via zinc reaction revealed mean background levels, accuracy, and sensitivity comparable to those obtained by our conventional hydrogen reaction lines. Zinc line reduction at the CIRCE laboratory represents an effective and powerful alternative to the conventional hydrogen reduction, ensuring higher sample throughput with lower costs at a comparable performance level. © 2008 by the Arizona Board of Regents on behalf of the University of Arizona.

A new AMS system has been installed at Center for Isotopic Research on Cultural and Environmental Heritage (CIRCE) in Caserta/ Italy. It was built by National Electrostatics Corp. Middleton, WI/USA, and arrived at CIRCE in the middle of January 2005; the installation and the acceptance tests were completed at the end of February 2005. At first, the system is intended for Radiocarbon AMS; for future Al and Be AMS only minor upgrades are needed and the magnets are designed to bend heavy isotopes like Uranium. Concerning Radiocarbon AMS, the first measurements of unknown samples, prepared in the internal preparation laboratory, already yielded results of high precision and C-14 dating is in full swing. Summary Background It is unclear whether radial compared with femoral access improves outcomes in unselected patients with acute coronary syndromes undergoing invasive management. Methods We did a randomised, multicentre, superiority trial comparing transradial against transfemoral access in patients with acute coronary syndrome with or without ST-segment elevation myocardial infarction who were about to undergo coronary angiography and percutaneous coronary intervention. Patients were randomly allocated (1:1) to radial or femoral access with a web-based system. The randomisation sequence was computer generated, blocked, and stratified by use of ticagrelor or prasugrel, type of acute coronary syndrome (ST-segment elevation myocardial infarction, troponin positive or negative, non-ST-segment elevation acute coronary syndrome), and anticipated use of immediate percutaneous coronary intervention. Outcome assessors were masked to treatment allocation. The 30-day coprimary outcomes were major adverse cardiovascular events, defined as death, myocardial infarction, or stroke, and net adverse clinical events, defined as major adverse cardiovascular events or Bleeding Academic Research Consortium (BARC) major bleeding unrelated to coronary artery bypass graft surgery. The analysis was by intention to treat. The two-sided α was prespecified at 0·025. The trial is registered at ClinicalTrials.gov, number NCT01433627. Findings We randomly assigned 8404 patients with acute coronary syndrome, with or without ST-segment elevation, to radial (4197) or femoral (4207) access for coronary angiography and percutaneous coronary intervention. 369 (8·8%) patients with radial access had major adverse cardiovascular events, compared with 429 (10·3%) patients with femoral access (rate ratio [RR] 0·85, 95% CI 0·74-0·99; p=0·0307), non-significant at α of 0·025. 410 (9·8%) patients with radial access had net adverse clinical events compared with 486 (11·7%) patients with femoral access (0·83, 95% CI 0·73-0·96; p=0·0092). The difference was driven by BARC major bleeding unrelated to coronary artery bypass graft surgery (1·6% vs 2·3%, RR 0·67, 95% CI 0·49-0·92; p=0·013) and all-cause mortality (1·6% vs 2·2%, RR 0·72, 95% CI 0·53-0·99; p=0·045). Interpretation In patients with acute coronary syndrome undergoing invasive management, radial as compared with femoral access reduces net adverse clinical events, through a reduction in major bleeding and all-cause mortality. (7)Be decays via electron capture and therefore its half-life is expected to depend on the electron density at the nucleus. We measured the (7)Be half-life in palladium, tungsten, zirconium and tantalum metals, in order to investigate the influence of the quasi-free electrons in metals on the probability of electron capture. The (7)Be samples were obtained implanting a 4.85 MeV pure radioactive (7)Be ion beam. The (7)Be half-life was determined measuring the 478 keV gamma decay following the electron capture by means of HPGe detectors. In order to reduce systematic errors, we planned to perform independent measurements in three different laboratories: Naples (Italy), Bochum (Germany) and Debrecen (Hungary). On the basis of the first results, we do not find a (7)Be half-life change within the experimental errors of 0.4%. RI Fulop, Zsolt/B-2262-2008

For improved cross-section measurements of the reaction 12C(α,γ)16O in inverted kinematics, a recoil separator ERNA is developed at the 4MV Dynamitron tandem accelerator in Bochum to detect directly the 16O recoils with high efficiency. Due to the emission of the capture γ-rays, the kinematically forward directed 16O recoils are described by an angle and energy spread. Non-radiative transitions in nuclear capture reactions between light nuclei play a relevant role in stellar nuclear astrophysics, where nuclear processes occur at typical energies from tens to hundreds of keV. At higher energies, instead, the E0 contributions may be shadowed by more intense transitions. The experimental study of E0 transitions requires a specific detection setup, able to uniquely identify events where an electron-positron pair is produced. A compact ΔE-E charged-particle spectrometer based on two silicon detectors has been designed to be installed in the jet gas target chamber of the recoil mass separator ERNA (European Recoil separator for Nuclear Astrophysics) at the CIRCE laboratory of Caserta, Italy. The detector design, its performances and the first foreseen applications are described.

For the electron capture of Be-7 in the metallic environments Pd and In the Be-7 half-life was observed to increase by 0.9 +/- 0.2 and 0.7 +/- 0.2%, respectively, while in the insulator Li2O it was unchanged within experimental error (all samples cooled to T = 12K). The observations are consistent with the predictions of the Debye plasma model applied to the quasi-free electrons in the metals.

The fusion reaction 12C(12C,p)23Na has been studied from E=2.00 to 4.00 MeV by particle spectroscopy. The data reveal broad resonances above E=3.00 MeV and are compatible with previously reported resonance structure around E=2.1 MeV. The data were limited at low energies by low count rates as well as possible background contributions. This experiment extends the previously achieved low-energy measurement by charged particle spectroscopy to 2 MeV, which corresponds to the high-energy side of the astrophysically relevant temperature. Present knowledge of level structures and nonresonant contribution cannot explain the results of the present experiment, which may change the 12C+12C reaction rate significantly. Despite the progress decreasing the low-energy limit, any extrapolation into the astrophysical energy range remains highly uncertain based on available experimental data. [ABSTRACT FROM AUTHOR]

Centre for Isotopic Research on Cultural and Environmental heritage (CIRCE) has, recently, obtained some promising results in testing the feasibility of mortar radiocarbon dating by means of an ad hoc developed purification procedure (CryoSoniC: Cryobraking, Sonication, Centrifugation) applied to a series of laboratory mortars. Observed results encouraged CryoSoniC accuracy evaluation on genuine mortars sampled from archeological sites of known or independently constrained age (i.e., other 14C dates on different materials). In this study, some 14C measurements performed on genuine mortars will be discussed and compared with independently estimated (i.e., radiocarbon/archaeometrical dating) absolute chronologies of two Spanish sites. Observed results confirm the agreement of the CryoSoniC mortar dates with the archaeological expectations for both examined cases. Several authors reported the possibility of obtaining accurate radiocarbon dates of mortar matrices by analyzing lime lumps: binder-related particles of different sizes exclusively composed of calcium carbonate. In this paper, preliminary data for the absolute chronology reconstruction of the Basilica of the cemetery complex of Ponte della Lama (Canosa di Puglia, Italy) based on lime lumps will also be discussed. Dating accuracy will be quantified by comparing 14C data on mortar lime lumps from a funerary inscription of known age found near the Basilica, in the same study site. For this site, a comparison between absolute chronologies performed by bulk and CryoSoniC purified lime lumps, and charcoal incased in mortars (when found) will also be discussed. Observed results for this site provide evidence of how bulk lime lump dating may introduce systematic overestimations of the analyzed sample while CryoSoniC purification allows accurate dating. © 2012 Elsevier B.V. Background Identification of high-risk patients with ST-segment elevation acute myocardial infarction (STEMI) is of the utmost importance for adequate patient stratification and evaluation of additive treatments. However, there is no consensus on the optimal definition of high-risk patients. Methods We therefore compared 5 scoring systems in the assessment of the risk of 30-day mortality in 3214 patients with STEMI treated with primary percutaneous coronary intervention (PCI). Results Clinical scores showed a large variability in risk stratifying patients. Identification of high-risk patients ranged from 15% (PAMI score≥9) to 66% (McNamara definition). McNamara, Antoniucci and Brodie definitions had the best sensitivity (0.87–0.88 and 95% confidence intervals (CI) ranging from 0.82–0.93) while PAMI≥9 had the best specificity (0.87 with 95% CI of 0.86–0.88), while its sensitivity was quite low (0.42). In a sample size simulation of a trial aimed at demonstrating a 33% difference in 30-day mortality between two hypothetical treatments, the number of STEMI patients needed to be screened varied from 4712 for the Brodie definition to 9038 for the PAMI≥9 score. Conclusions There is a large variability in risk stratification, sensitivity, specificity and predictive values among different scoring systems. These considerations should be taken into account when designing randomised trials.

The Ansanto Valley (southern Italy) is characterized by hydrothermal phenomena, with volcanic gas emissions arising from some vents. In the 1 st millennium BC, a sanctuary dedicated to the goddess Mephitis was built but later destroyed by landslides in the valley. During archaeological excavations in the 1950s, many items were found including wooden artifacts, preserved thanks to the imbibition and subsequent mineralization of the wood tissues due to the gas emissions. Radiocarbon dating of these objects is underway at CIRCE (Centre for Isotopic Research on Cultural and Environmental Heritage), in Caserta, Italy. Unfortunately, 2 main problems arise in dating these materials. The first is possible fossil dilution caused by the CO2 emitted from the nearby volcanic vents, which could affect the trees of the valley and also the archaeological materials. In order to determine the magnitude of the fossil dilution in the area, 14C measurements were performed on contemporaneous wood cored from 2 oak trees growing near the vents. 14C values measured in these samples confirmed the presence of a strong fossil dilution in the Ansanto Valley. The second problem is the restoration that the objects underwent during the last century (mostly by using modern organic substances). To investigate suitable pretreatment procedures for removing the restoration materials from the archaeological findings, contemporaneous wood was also analyzed. The wood of trees from the Ansanto Valley and from a distant village (unaffected by the Ansanto fossil dilution) were submitted to the same restoration process applied to the archaeological artifacts, followed by an “artificial weathering” process. Some archaeological materials were also tested for the removal of restoration materials. We subjected the artificially aged trees and the archaeological samples to different chemical processes. Here, we present the results of these processes. Almost all methods turned out to be suitable for the contemporaneous wood, while the results for the archaeological samples remain uncertain. For this reason, more tests are needed, concerning the “artificial weathering,” the restoration, and the chemical procedure for removing the consolidation materials.

Evidence of prehistoric dentistry has been limited to a few cases, the most ancient dating back to the Neolithic. Here we report a 6500-year-old human mandible from Slovenia whose left canine crown bears the traces of a filling with beeswax. The use of different analytical techniques, including synchrotron radiation computed micro-tomography (micro-CT), Accelerator Mass Spectrometry (AMS) radiocarbon dating, Infrared (IR) Spectroscopy and Scanning Electron Microscopy (SEM), has shown that the exposed area of dentine resulting from occlusal wear and the upper part of a vertical crack affecting enamel and dentin tissues were filled with beeswax shortly before or after the individual's death. If the filling was done when the person was still alive, the intervention was likely aimed to relieve tooth sensitivity derived from either exposed dentine and/or the pain resulting from chewing on a cracked tooth: this would provide the earliest known direct evidence of therapeutic-palliative dental filling.

Background: The purpose of this study is to present data on the effects of pre-hospital electrocardiogram (PH-ECG) on the outcome of ST elevation myocardial infarction (STEMI) patients treated with percutaneous coronary angioplasty (PCI) included in a registry undertaken in the Italian region of Lombardy. Pre-hospital 12-lead electrocardiogram is recommended by current guidelines in order to achieve faster times to reperfusion in patients with STEMI. Methods: The registry includes 3901 STEMI patients who underwent primary PCI over an 18-month period. Results: Mean age was 63 ± 12 years. Admission through the emergency medical system (EMS) occurred in 1603 patients (40%): they were older, more frequently had previous MI, TIMI flow = 0 at entry and were more frequently in Killip class >1 than patients who were not admitted through the EMS. Among the patients admitted through the EMS, PH-ECG was obtained in 475 patients (12%). These patients had less frequently an anterior MI, but more frequently had absence of TIMI flow at entry than patients whose ECG was not teletransmitted. Moreover, they had a significantly shorter first medical contact-to-balloon time and a trend toward a lower 30-day death rate (5.3% vs 7.9 %, p = 0.06). However, only patients in Killip class 2-3 had a significantly lower mortality when the diagnostic ECG was transmitted, whereas no difference was found in Killip class 1 or Killip class 4 patients. Conclusions: In this registry, PH-ECG significantly decreased first medical contact-to-balloon time. Attempts to achieve faster reperfusion times should be undertaken, as this may result in improved outcome, particularly in patients with mild to moderate symptoms of heart failu

The operation of Nuclear Power Plants and atmospheric tests of nuclear weapons performed in the past, together with production, transport and reprocessing of nuclear fuel, lead to the release into the environment of a wide range of radioactive nuclides, such as uranium, plutonium, fission and activation products. These nuclides are present in the environment at ultra trace levels. Their detection requires sensitive techniques like AMS (Accelerator Mass Spectrometry). In order to perform isotopic ratio measurements of the longer-lived actinides, e.g., of 236U relative to the primary 238U and various Pu isotopes relative to 239Pu, an upgrade of the CIRCE accelerator (Center for Isotopic Research on Cultural and Environmental Heritage) in Caserta, Italy, is underway. In this paper we report on the results of simulations aiming to define the best ion optics and to understand the origin of possible measurement background. The design of a high resolution TOF-E (Time of Flight-Energy) detector system is described, which will be used to identify the rare isotopes among interfering background signals.

Human bones recovered from the archaeological site of Pontecagnano (Salerno, Italy) have been studied to reconstruct the diet of an Etrurian population. Two different areas were investigated, named Library and Sant’ Antonio, with a total of 44 tombs containing human skeletal remains, ranging in age from the 8th to the 3rd century B.C. This time span was confirmed by 14C dating obtained using Accelerator Mass Spectrometry (AMS) on one bone sample from each site. Atomic Absorption Spectrometry (AAS) was used to extract information about the concentration of Sr, Zn, Ca elements in the bone inorganic fraction, whilst stable isotope ratio measurements (IRMS) were carried out on bone collagen to obtain the δ13C and δ15N. A reliable technique has been used to extract and separate the inorganic and organic fractions of the bone remains. The 3He(α, γ)7Be reaction plays an important role in the interpretation of the results of the solar neutrino experiments, since the estimate of the oscillation parameters relies on the solar neutrino spectrum, calculated by solar models. The high energy component in this spectrum is mainly produced by the decay of 7Be and 8B. The uncertainty in the 3He(α, γ) 7Be cross section is also one of the largest contributions to the uncertainty on the predicted primordial 7Li abundance in Big Bang Nucleosynthesis calculations. Previous measurements of the 3He(α, γ)7Be cross section have been performed detecting the capture γ-rays or, alternatively, measuring the activity of the synthesized 7Be. While the results of the two different approaches agree on the energy dependence of the astrophysical S factor, they disagree in the extrapolated S34(0) value at a 3σ level. A novel approach uses the European Recoil separator for Nuclear Astrophysics (ERNA) to detect directly the 7Be ions produced in the reaction and, additionally, the coincident detection of the capture γ-rays. © 2006 American Institute of Physics.

For improved cross-section measurements of the reaction 12C(α,γ)16O in inverted kinematics, a recoil separator ERNA is developed at the 4MV Dynamitron tandem accelerator in Bochum to detect directly the 16O recoils with high efficiency. The 16O recoils are produced by the 12C projectiles in a windowless 4He gas target. We report on the pressure profile of the gas target, the beam suppression by the separator, and the first observation of the 16O recoils at selected energies. © 2003 Elsevier B.V.
