Abstract Background Ample evidence indicates the efficacy of serotonin type 3 (5-HT3) receptor antagonists in the treatment of patients with diarrhea-predominant irritable bowel syndrome (IBS-D). Mirtazapine is an atypical antidepressant with a well-known 5-HT3 receptor antagonist property. This study, therefore, was undertaken to investigate whether compared to placebo, mirtazapine would be efficacious and safe in the treatment of patients with IBS-D. Methods From November 2019 until July 2020, 67 patients meeting Rome IV criteria for IBS-D were randomized in a double-blind fashion into either the mirtazapine treatment group (n = 34) or the placebo treatment group (n = 33). Patients started with mirtazapine 15 mg/day at bedtime for one-week; after which the dose was increased to 30 mg/day for an additional 7-week. Outcomes included changes in the total IBS symptom severity score (IBS-SSS), Hospital anxiety and depression scale score (HADS), and IBS Quality of Life. Additionally, changes in the diary-based symptoms scores including pain, urgency of defecation, bloating, stool frequency, and stool consistency based on the 7-point Bristol Stool Form Scale (BSFS), and a number of days per week with pain, urgency, diarrhea, or bloating, once during the 1-week run-in period, and once during the last week of treatment were recorded. Results All analyses were performed on an Intention-to-Treat (ITT) analysis data set. The results showed compared to placebo, mirtazapine is more efficacious in decreasing the severity of IBS symptoms (P-value = 0.002). Further, at the end of the treatment period, all diary-derived symptoms except bloating showed significantly more improvement in the mirtazapine-treated subjects compared to the placebo-treated subjects. While was well-tolerated, mirtazapine also significantly improved the patients’ quality of life (P-value = 0.04) and anxiety symptoms (P-value = 0.005). Conclusions Overall, mirtazapine seems to have a potential benefit in the treatment of patients with IBS-D, particularly those with concomitant psychological symptoms. However, further studies are warranted to determine whether these findings are replicated. 104. Assessing the Impact of Climate Change on the Inflow on Zayandehrood Dam

In the present study, the SWAT hydrological model was developed for the upstream of the Zayandehrood dam to evaluate the inflow to this dam. Accordingly, after entering the meteorological and hydrometric information of the region, the runoff simulation was performed. Due to the high volume of entrances to the Zayandehrood Dam, Shahrokh Castle hydrometric stations were selected as the base station for calibration and validation during the statistical period of 1990-2015. After hydrological simulation and accuracy of results, climate prediction was performed using the fifth model of the climate change for the RCP scenarios. According to the forecast, by using climate change models, the temperature could be assumed to increase in all models and the highest rate of increase would occur under the RCP 8.5 climate scenario. After evaluating climate change in different diffusion scenarios, the runoff of the basin was simulated in the SWAT model. The simulation results of runoff in the catchment area showed that although the amount of rainfall was increased in the region, increasing the temperature had a greater effect, reducing the amount of runoff in the basin. Based on the results of climate change, hydrological simulation was performed using the SWAT model. The results showed that the effect of diffusion scenarios in the region was different, causing an increase in temperature and precipitation. The highest increase was observed in the RCP8.5 scenario, which was consistent with the nature of this emission scenario, with the highest emission of greenhouse gases and carbon dioxide. Then, the evaluation of the hydrological model was done; the results showed that although the amount of rainfall in the region had been increased, the increase in temperature of this basin had a greater effect and efficiency in reducing the amount of runoff.

Abstract Background Numerous studies have shown that QT dispersion (QTd) can be a suitable criterion for risk assessment of arrhythmia in patients with congenital heart disease. Pulmonary arterial hypertension (PAH) increases the risk of cardiac arrhythmia by changing ventricular repolarization homogeneity. In this study, we assessed QTd changes after PDA device closure and the effect of PAH on these changes. Methods Between October 2018 and March 2021, 97 patients (48 males; 49 females; mean age 31.36 ± 4.26 months; range 3 months to 14 years) who satisfied the primary inclusion criteria and did not meet the exclusion criteria and underwent PDA device closure intervention were included in the study. Echocardiography was performed before the procedure. QT corrected (QTc), and QTd and PR intervals were measured according to the patients’ standard 12‐lead ECGs in two periods, preoperative (1 day) and after (3 months). Results In the general group, QTc and QTd decreased significantly after PDA closure. Based on our classification of the patients in two groups of high PAP and normal PAP, the three parameters QTc, QTd, and PR interval were assessed separately in the two groups. All three parameters decreased significantly in the normal PAP and high PAP groups. Conclusions However, a left‐to‐right shunt through the patent ductus arteriosus can affect ventricular repolarization; this effect seems to be particularly more significant when there is pulmonary hypertension.

Promoting water productivity in food production is one of the main issues in different countries of the world, especially in low-water countries like Iran. In the present situation, measurement and analysis of productivity indices of scarce water input in agricultural sector in Iran is of special importance due to the quantitative and qualitative limitation of this valuable material. Therefore, the purpose of the present study is to measure and compare the physical and economic productivity of irrigation water use in two traditional and modern irrigation systems for major crops (Wheat, Barley, Rapeseed, Alfalfa and Forage Corn) in Shahriar. In this study, the CPD, BPD and NBPD were used to calculate of agricultural water productivity. According to the results of the study, in both irrigation systems, the priority of crop cultivation to the second rank is based on CPD index Forage Corn and Alfalfa, respectively. Also based on both BPD and NBPD indices in modern irrigation system are Rapeseed and Wheat respectively and in traditional irrigation system Rapeseed and Alfalfa, respectively.

Kichi is a local sweet bread that is cooked in the west of Iran, especially in Chaharmahal and Bakhtiari Province. This product is traditionally produced and has a low shelf life and hard texture. Therefore, the aim of this study was to investigate the effect of xanthan gum (0, 0.1, 0.3 and 0.5%) to improve the quality of Kichi sweet bread. Also mean size area, porosity, fractal dimension, and color parameters of Kichi bread were evaluated. The results showed that addition of xanthan led to improving WHC. The results of the texture analysis showed that softer samples were obtained in the low concentrations of xanthan gum (0.1% based on flour) and gumminess in this sample was significantly less than other samples (p < /em>

Background and purpose: Social capital is one of the factors affecting the health of diabetic patients which will also affect their quality of life. The purpose of this study was to investigate the relationship between social capital, psychological well-being, and quality of life in diabetes patients. Materials and methods: A cross-sectional study was conducted in 275 diabetic patients attending Qaemshahr Razi Hospital in north of Iran. They were selected via convenience sampling. Data were collected using a demographic checklist, the Social Capital Questionnaire (SCQ), Diabetes Quality of Life questionnaire (DQOL), and World Health Organization Well-Being Index. Data were analyzed in SPSS V22. Results: The mean values for social capital, psychological well-being, and quality of life were 103.74 ± 19.56, 13.19 ± 5.47, and 43.59 ± 7.2, respectively. Moving object detection in a given video sequence is a pivotal step in many computer vision applications such as video surveillance. Robust Principal Component Analysis (RPCA) performs low-rank and sparse decomposition to accomplish such a task when the background is stationary and the foreground is dynamic and relatively small. A fundamental issue with the RPCA is the assumption that the low-rank and sparse components are added at each pixel, whereas in reality, the moving foreground is overlaid on the background. We propose the masked decomposition (i.e. an overlaying model) where each element either belongs to the low-rank or the sparse component, decided by a mask. We introduce the Masked-RPCA (MRPCA) algorithm to recover the mask (hence the sparse object) and the low-rank components simultaneously, via a non-convex formulation. An adapted version of the Douglas-Rachford splitting algorithm is utilized to solve the proposed formulation. Our experiments using real-world video sequences show consistently better performance for both cases of static and dynamic background videos compared to RPCA and its variants based on the additive model. Additionally, we show that utilizing non-convex priors in our formulation leads to improved results without any added complexity compared to a relaxed formulation using convex surrogates and methods based on the additive model.

113. Effects of cinnamon extract on complications of treatment and eradication of Helicobacter pylori in infected people

Introduction: Treatment of Helicobacter pylori has various side effects like antibiotic resistance. The purpose of this study was to evaluate the effects of cinnamon extract on complications of treatment and eradication of H. pylori in infected people. Methods: In this randomized clinical trial, a total of 98 eligible healthy and H. pylori-infected patients approved by esophageal endoscopy were selected. The cinnamon group received multi-drug treatment including clarithromycin, amoxicillin and pantoprazole as well as a cinnamon extract capsule. The control group received multi-drug treatment and a 40 mg starch capsule. In order to analyze the cinnamon extract efficacy, the urea breath test (UBT) was performed 3 months after the start of treatment. Clinical symptoms were evaluated by a questionnaire at the beginning (day of 0), 7 days and 14 days after starting treatment. Results: The clinical symptoms such as nausea, vomiting, diarrhea, constipation, blurred vision, headache, metallic flavor, epigastric pain, burp, and appetite were significantly reduced in the cinnamon group (P < 0.05). The odds ratio exhibited a higher eradication rate of H. pylori in the cinnamon group (73.47% in the cinnamon group compared to 53.06% in the control group) (P = 0.036). Conclusion: Cinnamon as assisted therapy is able to alleviate the disease and reduce the complications of H. pylori treatment.

Interruption of the right pulmonary artery is a very rare anomaly which can be associated with other congenital heart lesions or can occur in isolation. Clinical presentations of the unilateral interruption of a pulmonary artery are varied including pulmonary hypertension, recurrent infection, dyspnea, exercise intolerance, hemoptysis, and chest pain. Less commonly, patients may be asymptomatic. Diagnosis of this anomaly is made by echocardiography and CT angiography as well as cardiac MRI. Treatment options are medical versus surgical management and often recommended in symptomatic patients with pulmonary hypertension, recurrent infection, and hemoptysis. Herein, we describe a very rare case of right pulmonary artery originating from the right subclavian artery in a 12-day-old neonate.

Background: Breast cancer (BC) is the leading cause of cancer death in women. The current study is designed to evaluate the association of lipid profiles, FBS, and body mass index (BMI) with BC recurrence and metastasis. Materials and Methods: This is a case–control study on estrogen receptor-positive BC patients in Isfahan Province, Central Iran, between 2008 and 2020. The control group was patients who had no evidence of recurrence or metastasis at least 1 year after the end of chemotherapy and hormone therapy. The case group was patients with evidence of metastasis or recurrence within 1 year after the end of chemotherapy and hormone therapy. Fasting blood sugar (FBS), total cholesterol (Chol), triglyceride (TG), high-density lipoprotein (HDL), and low-density lipoprotein (LDL) were measured before treatment, after chemotherapy, and after hormone therapy as well as BMI in the case and control groups. Results: There were 108 patients in the case and 119 patients in the control group with a mean age of 50.72 ± 13.26 and 51.91 ± 11.79, respectively. There were no meaningful differences between the case and control groups regarding serum FBS, Chol, TG, HDL, LDL, and BMI. Conclusion: We found no association between serum FBS, lipid profile, and BMI at initial diagnosis and BC recurrence or metastasis.

A 9-year-old girl with signs and symptoms of acute toxic myocarditis and cardiogenic shock with elevated cardiac enzymes was admitted to the Critical Care Unit (CCU) of our hospital with an ejection fraction of 25%. The patient was managed with supportive care and the administration of polyvalent antivenom and inotropes, and after 8 days, she was discharged without any complication with normal ejection fraction. Toxic myocarditis can be a result of scorpion envenomation. After two months of follow-up, the patient recovered completely and medications were discontinued.

Hydatid disease is a parasitic infection caused by Echinococcus granulosus. Cardiac involvement is rare especially without liver and lungs tissue involvement. We describe a 12-year-old male patient referred to Mofid Children's Hospital, Tehran, Iran in Jul 2020 due to chronic pericardial effusion and suspected tuberculosis infection from Afghanistan. Echocardiography revealed a cystic lesion in the interventricular septum. Thoracic and abdominal computed tomography showed no similar cystic lesion in the lungs and liver. The patient underwent open-heart surgery for cystectomy and medical treatment with albendazole. Histological examination confirmed hydatid cyst diagnosis. The patient was discharged in good condition and oral albendazole was continued.

Entropy-driven directed translocation of a flexible polymer through a cone-shaped channel is studied theoretically and using computer simulation. For a given length of the channel, the effective force of entropic origin acting on the polymer is calculated as a function of the apex angle of the channel. It is found that the translocation time is a non-monotonic function of the apex angle. By increasing the apex angle from zero, the translocation time shows a minimum and then a maximum. Also, it is found that regardless of the value of the apex angle, the translocation time is a uniformly decreasing function of the channel length. The results of the theory and the simulation are in good qualitative agreement.

Multiple sclerosis (MS) is a demyelinating inflammatory disorder of the central nervous system (CNS). Besides the vital role of T cells, other immune cells, including B cells, innate immune cells, and macrophages (MФs), also play a critical role in MS pathogenesis. Tissue-resident MФs in the brain’s parenchyma, known as microglia and monocyte-derived MФs, enter into the CNS following alterations in CNS homeostasis that induce inflammatory responses in MS. Although the neuroprotective and anti-inflammatory actions of monocyte-derived MФs and resident MФs are required to maintain CNS tolerance, they can release inflammatory cytokines and reactivate primed T cells during neuroinflammation. In the CNS of MS patients, elevated myeloid cells and activated MФs have been found and associated with demyelination and axonal loss. Thus, according to the role of MФs in neuroinflammation, they have attracted attention as a therapeutic target. Also, due to their different origin, location, and turnover, other strategies may require to target the various myeloid cell populations. Here we review the role of distinct subsets of MФs in the pathogenesis of MS and different therapeutic agents that target these cells.

Very large databases are required to store massive amounts of data that are continuously inserted and queried. Analyzing huge data sets and extracting valuable pattern in many applications are interesting for researchers. We can identify two main groups of techniques for huge data bases mining. One group refers to streaming data and applies mining techniques whereas second group attempts to solve this problem directly with efficient algorithms. Recently many researchers have focused on data stream as an efficient strategy against huge data base mining instead of mining on entire data base. The main problem in data stream mining means evolving data is more difficult to detect in this techniques therefore unsupervised methods should be applied. However, clustering techniques can lead us to discover hidden information. In this survey, we try to clarify: first, the different problem definitions related to data stream clustering in general; second, the specific difficulties encountered in this field of research; third, the varying assumptions, heuristics, and intuitions forming the basis of different approaches; and how several prominent solutions tackle different problems. Introduction and purpose: Job burnout is a kind of psychological distress that results from various occupational factors and leads to the deterioration of job performance. This condition is usually characterized by weakness, disappointment, and nonachievement of career goals. Regarding this, the present study aimed to compare the frequency of burnout syndrome between general family and non-family physicians in Mazandaran province, Iran, in 2018. Methods: This cross-sectional study was conducted on 153 urban family physicians, 158 rural family physicians, and 295 non-family physicians (i.e., a total of 606 cases) selected using simple random sampling technique. In this study, Maslach Burnout Questionnaire was used, as the main tool for measuring the three dimensions of emotional exhaustion, depersonalization, and personal empowerment. Study participants included two groups of family general practitioners with more than two years of experience working in all shifts as a family physician, as well as non-family general practitioners working in health centers and offices affiliated to mazandaran university of medical sciences. General practitioners working in the hospital and staffing plan were excluded from the study. One-way ANOVA was used to compare the mean values and the mean differences of burnout dimensions at two levels of urban and rural family physicians and those outside the family physician plan. 129. The Effects of Topical Ketamine in Reducing Venipuncture Pain

Background: Different pharmacological methods have been introduced for managing venipuncture pain. The aim of this study was to evaluate the effect of topical ketamine in reducing the pain of venipuncture. Methods: In this randomized clinical trial study, 200 patients with indication for venipuncture were enrolled. Patients were randomly divided into two groups. In the first group, the venipuncture site was covered with topical ketamine and in the second group, with placebo. After 45 minutes, the site was examined for numbness, and when complete numbness achieved, professional nurses performed the venipuncture. Then, pain severity [by visual analog scale (VAS)] and side effects were recorded and compared between the groups. Findings: The mean age of patients was 45.15 ± 10.36 years and 50% were men. There was no statistically significant difference between the two groups in terms of age and sex. VAS was significantly lower in the ketamine group than in the control group (P < 0.001); but the side effects such as redness, hives, itching, and burning were significantly less in control group (P < 0.050). The mean duration of complete numbness in the ketamine group was 58.20 ± 9.48 hours (P < 0.050). Conclusion: Topical ketamine is effective in reducing the pain of venipuncture in adult. To expect a full effect, ketamine cream should be applied at least 60 minutes prior to cannulation.

130. Estimation of Static and Dynamic Demand Function of Household Water in Qazvin Province and Review of the Rate of Change in Consumer Behavior Over Time

In this research, the demand function of drinkable water in Qazvin Province was estimated using dynamic and static methods. The required data were collected from the data of the provinces of Qazvin in the time period (1996-2016) and collected by referring to the statistical system of Statistics Center of Iran and the provincial planning and budget organization of Qazvin Province. The explanatory variables used in the model include household income, temperature (minimum and maximum), urban population, water price, rainfall, number of subscribers.The method used to estimate the static model, generalized least squares, and in the dynamic model, is a two-step generalized moment method. The results showed that the water price coefficient in the static and dynamic model is -0.217 and -0.19, respectively, which is a negative sign of the correctness of the demand law and less than one indicative of the low elasticity of water. The variable coefficient of household income in the static and dynamic model was 0.2 and 0.15, respectively, which positive and less than one, respectively, indicating the normality and necessity of water in the drinkable sector. In relation to the price of other goods, the coefficient was estimated in both negative models were -0.72 and -0.9, respectively, which indicates that water is a complementary product. Finally, the value of the variable coefficient was 0.5, which indicates that the demand for water is 0.5 times the demand for water last year.

131. Prevalence and Correlates of Cigarette Smoking in Adolescent Psychiatric Inpatients in Iran: A Cross-Sectional Study and Narrative Review

Context: Smoking in adolescence is a worldwide health problem. Understanding the prevalence of smoking and its clinical correlates in adolescent inpatients is useful for mental health staff to plan effective programs to reduce its detrimental consequences. In this study, we investigated the prevalence and correlates of cigarette smoking and its sociodemographic and clinical correlates. We performed a narrative review of cigarette smoking prevalence among adolescents as well as a descriptive study in inpatient adolescents in Iran. Evidence Acquisition: This was a multicentric, cross-sectional, and hospital-based survey. In total, 82 patients (male/female: 66.9% vs. 33.1%) were consecutively recruited from 5 child and adolescent inpatient psychiatric wards of Iran. The study data were collected from patients, their family members and patients’ medical records. The descriptive statistics, Pearson’s chi-squared test, and univariate logistic regression analyses were employed to analyze the obtained data. Additionally, we added a narrative review to understand the prevalence rate of smoking and its clinical correlates in adolescent inpatients. Results: The prevalence rate of smoking was 30% in our study sample (24.5% in boys and 39.5 % in girls). There was a significant association between smoking with family size, history of suicide attempt, physical illness, drug misuse in patients, and alcohol or drug misuse in family members. The prevalence of mood disorders in the nonsmokers and smokers were 37.5% and 62.5%, respectively. Overall, 26.6% of the subjects had a history of suicide attempt of whom, 52.9% were smokers. Conclusions: Results of this study about the association between smoking with family size, history of suicide attempt, physical illness, drug misuse, and other related factors, call for both careful screening of smoking and specially designed integrated psychiatric/smoking treatments in these patients.

Brand ambidexterity strategies help organizations improve their capabilities and performance and simultaneously discover new opportunities. The purpose of this study is to investigate the effects of brand ambidexterity strategies on brand commitment through brand’s performance, image and reputation. The statistical population of this research were the users of Pishgaman Company. Random sampling method was used and 300 acceptable questionnaires were collected. Data analysis was carried out using the PLS3 software and the results show that exploratory and exploitative strategies have positive and significant impact on brand performance. Also, the impact of brand’s performance on its reputation and image, and brand reputation on brand commitment are positive and significant. Other findings show that the impact of brand performance on brand commitment, brand image on brand reputation and brand image on brand commitment are not significant. Moreover, results indicate that brand performance has a mediating role in the relationship between brand ambidexterity and brand commitment, and reputation has a mediating role in the relationship between brand performance and brand commitment. Nevertheless, the mediating role of brand image in the relationship between brand performance and brand commitment has not been confirmed.

Background and purpose: In recent decades, various efforts have been made to replace synthetic materials with natural disinfectants that have less disadvantages and lead to similar or even better results. The present study was conducted to compare the effect of Artemisia aucheri and Zataria multiflora as natural disinfectants with those of Deconex and sodium hypochlorite as two industrial disinfectants on Candida albicans. Materials and methods: This experimental study was conducted in 128 resin acrylic dentures. After the inoculation of C. albicans on each denture, they were divided into four groups, each containing 32 dentures. Deconex, Sodium hypochlorite, Artemisia aucheri, and Zataria multiflora were administered in each group in four densities (0.1, 0.01, 0.001, and 0.0001). Background: Helicobacter pylori (H. pylori) has various strains associated with human infections. H. pylori, H. heilmannii, and H. felis are the most common strains in humans. H. pylori is associated with several human diseases such as chronic gastritis, peptic ulcer, mucous membrane lymphoma, and gastric adenocarcinoma. This study aimed to determine the prevalence rates of H. felis and H. heilmannii and the effect of co-infection with H. pylori in gastric biopsy specimens of patients. Methods: Totally, 80 gastric biopsy specimens were taken by a physician from the patients referred to Shahid Beheshti Hospital, Hamadan City, Iran. PCR test was used to confirm the presence of H. pylori in samples that had positive rapid urease tests. Moreover, the ureB gene and ureA and ureB genes were used for H. heilmannii and H. felis, respectively. Results: Of the study patients, 61.5% were females, and 38.5% were males with a mean age of 37.8 years. Of 80 biopsies, 50% were H. pylori-positive, 53.8% were H. heilmannii-positive, but no H. felis was identified in any sample. Results indicate that smoking, having a history of gastrointestinal diseases, and taking certain medications can be risk factors for H. pylori. Conclusion: Any agent contributing to gastric mucosal damage can enhance the susceptibility to bacterial contamination. Overall, the results indicate a low probability of interactions between H. pylori, H. heilmannii, and H. felis.

Abstract Background Cultivated peanut (Arachis hypogaea) is an allotetraploid species whose ancestral genomes are most likely derived from the A-genome species, A. duranensis, and the B-genome species, A. ipaensis. The very recent (several millennia) evolutionary origin of A. hypogaea has imposed a bottleneck for allelic and phenotypic diversity within the cultigen. However, wild diploid relatives are a rich source of alleles that could be used for crop improvement and their simpler genomes can be more easily analyzed while providing insight into the structure of the allotetraploid peanut genome. The objective of this research was to establish a high-density genetic map of the diploid species A. duranensis based on de novo generated EST databases. Arachis duranensis was chosen for mapping because it is the A-genome progenitor of cultivated peanut and also in order to circumvent the confounding effects of gene duplication associated with allopolyploidy in A. hypogaea. Results More than one million expressed sequence tag (EST) sequences generated from normalized cDNA libraries of A. duranensis were assembled into 81,116 unique transcripts. Mining this dataset, 1236 EST-SNP markers were developed between two A. duranensis accessions, PI 475887 and Grif 15036. An additional 300 SNP markers also were developed from genomic sequences representing conserved legume orthologs. Of the 1536 SNP markers, 1054 were placed on a genetic map. In addition, 598 EST-SSR markers identified in A. hypogaea assemblies were included in the map along with 37 disease resistance gene candidate (RGC) and 35 other previously published markers. In total, 1724 markers spanning 1081.3 cM over 10 linkage groups were mapped. Gene sequences that provided mapped markers were annotated using similarity searches in three different databases, and gene ontology descriptions were determined using the Medicago Gene Atlas and TAIR databases. Synteny analysis between A. duranensis, Medicago and Glycine revealed significant stretches of conserved gene clusters spread across the peanut genome. A higher level of colinearity was detected between A. duranensis and Glycine than with Medicago. Conclusions The first high-density, gene-based linkage map for A. duranensis was generated that can serve as a reference map for both wild and cultivated Arachis species. The markers developed here are valuable resources for the peanut, and more broadly, to the legume research community. The A-genome map will have utility for fine mapping in other peanut species and has already had application for mapping a nematode resistance gene that was introgressed into A. hypogaea from A. cardenasii.

BackgroundMonitoring mental health outcomes has traditionally been based on heuristic decisions, often based on scarce, subjective evidence, making the clinical decisions made by professionals, as well as the monitoring of these diseases, subject to flaws. However, the digital phenotype, which refers to the analysis of data collected by measuring human behavior with mobile sensors and smart bracelets, is a promising tool for filling this gap in current clinical practice. ObjectiveThe objectives of this study are to develop the digital phenotyping of patients with alcohol use disorder and anxiety symptoms using data collected from a mobile device (ie, smartphone) and a wearable sensor (ie, Fitbit) and to analyze usability and patient satisfaction with the data collection service provided by the app. MethodsWe propose to conduct a study among a group of 60 participants split into two subgroups—experimental and control—of 30 participants each. The experimental group will be recruited by physicians from the Hospital Clínic de Barcelona, and the control group will be recruited on a volunteer basis through fliers and social media. All participants will go through pretraining to ensure technological capability and understanding of tasks, then each participant will download the HumanITcare app and will be given a wearable sensor (ie, Fitbit). Throughout the 4-month period, participants will be monitored on a range of factors, including sleep cycle, heart rate, movement patterns, and sociability. All data from the wearable sensors and the mobile devices will be saved and sent to the HumanITcare server. Participants will be asked to complete weekly questionnaires about anxiety, depression, and alcohol use disorder symptoms. Research assistants will ensure timely responses. The data from both sensors will then be compared to the questionnaire responses to determine how accurately the devices can predict the same symptoms. ResultsThe recruitment phase was completed in November 2019 and all the data were collected by the end of December 2019. Data are being processed; this process is expected to be completed by October 2020. ConclusionsThis study was created and conducted as a pilot study with the Hospital Clínic de Barcelona, with the purpose of exploring the feasibility of our approach. The study is focused on patients diagnosed with anxiety and alcohol use disorder, but participants were also monitored for depressive symptoms throughout the trial, although these were not part of the initial inclusion criteria. A limitation to our study was the exclusive use of Android smartphones over iOS devices; this could result in a potential selection bias, due to the accessibility and affordability of Android phones as opposed to iOS-based phones. Another limitation might be that reviews of usability and satisfaction could be confounded by factors such as age and familiarity. Summary: Stem cells can be maintained through symmetric cell divisions (SCDs) and asymmetric cell divisions (ACDs). How and when these divisions occur in vivo in vertebrates is poorly understood. Here, we developed a clonogenic cell tracing method that demonstrates the asymmetric distribution of transcription factors along with old and new DNA in mouse muscle stem cells during skeletal muscle regeneration. Combining single-cell tracking and artificial niches ex vivo, we show how cells switch from ACDs to SCDs, suggesting that they are not engaged in an obligate mode of cell division. Further, we generated SNAP-tagged histone H3-reporter mice and find that, unlike fly germline stem cells, differential fate outcomes are associated with a symmetric distribution of the H3.1 and H3.3 histone variants in mouse muscle stem cells. This versatile and efficient H3-SNAP labeling system will allow an investigation of mechanisms underlying the maintenance of epigenomic identity and plasticity in a variety of tissues. : Using SNAP-tagged histone H3-reporter mice and clonogenic tracing, Evano et al. show that muscle stem cells can perform symmetric and asymmetric cell divisions (SCDs; ACDs) in vivo, and switch from ACDs to SCDs ex vivo, with symmetric inheritance of H3.1 and H3.3. Alcohol (ethanol, EtOH) consumption during pregnancy can result in fetal alcohol spectrum disorders (FASDs), which are characterized by prenatal and postnatal growth restriction and craniofacial dysmorphology. Recently, cell-derived extracellular vesicles, including exosomes and microvesicles containing several species of RNAs (exRNAs), have emerged as a mechanism of cell-to-cell communication. However, EtOH's effects on the biogenesis and function of non-coding exRNAs during fetal development have not been explored. Therefore, we studied the effects of maternal EtOH exposure on the composition of exosomal RNAs in the amniotic fluid (AF) using rat fetal alcohol exposure (FAE) model. Through RNA-Seq analysis we identified and verified AF exosomal miRNAs with differential expression levels specifically associated with maternal EtOH exposure. Uptake of purified FAE AF exosomes by rBMSCs resulted in significant alteration of molecular markers associated with osteogenic differentiation of rBMSCs. We also determined putative functional roles for AF exosomal miRNAs (miR-199a-3p, miR-214-3p and let-7g) that are dysregulated by FAE in osteogenic differentiation of rBMSCs. Our results demonstrate that FAE alters AF exosomal miRNAs and that exosomal transfer of dysregulated miRNAs has significant molecular effects on stem cell regulation and differentiation. Our results further suggest the usefulness of assessing molecular alterations in AF exRNAs to study the mechanisms of FAE teratogenesis that should be further investigated by using an in vivo model.

141. Antidepressant Effect of Asperugo procumbens L. in Comparison with Fluoxetine: a Randomized Double Blind Clinical Trial

Background and objectives: Aseprugo procumbens is used as antianxety and depression in Iranian traditional medicine; however no clinical trial has been performed about these properties. The aim of this study was to evaluate the antidepressant efficacy and safety of hydroalcoholic extract of A. procumbens. Methods: In this double blind study, 30 outpatients with mild to moderate depressive disorder (according to DMS-IV-TR criteria who had a score of 18-25 on the Hamilton Depression Rating Scale (HDRS) were randomly divided in two group, 1) received 10 mg fluoxetine capsule, 2) received 1.2 g dried extract of A. procumbens capsule (6 mg flavonoid,) orally per day. Patients were followed up for 6 weeks; the recovery process was assessed by the HDRS in weeks 0, 2, 4 and 6. Results: A significant decrease in HDRS was observed in both groups after the fourth week. After the sixth week, fluoxetine had a greater anti-depressant activity than A. procumbens (p value =0.03). There were no significant differences in terms of adverse effects in both groups. Conclusions: The hydroalcoholic extract of A. procumbens can be considered as an effective and safe remedy for mild to moderate depressive disorder.

142. The Association between Polymorphism of Xrcc3 Gene and Colorectal Cancer: A Case-Control Study

Background: Colorectal cancer is the third cancer which results in death in western countries. Some of the risk factors of this cancer are age, inadequate diet, obesity, inactivity, genetic changes, etc. Considering the relationship between genetic changes with colorectal cancer is the subject of recent studies, and one of the subjects which have been focused is T241M polymorphism in Xrcc3 gene. Methods: In this case-control study, 90 patients with colorectal cancer and 83 healthy people were included. Genomic DNA was extracted from peripheral blood, and genotype distribution in the region of polymorphism was studied. By designing of primers, the considered area was amplified and genotyped using polymerase chain reaction-restriction fragment length polymorphism analysis (PCR-RFLP) method. Findings: We observed the association between T241M polymorphism and colorectal cancer. Besides, there was a meaningful relationship between aging and family history with this cancer. On the other hand, smoking was not associated with colon cancer, but it also showed a significant role in the incidence and metastasis of rectal cancer. Conclusion: The relationship between T241M polymorphism and increased risk of colorectal cancer, along with family history of cancer, old age, and smoking were reported as risk factors for colorectal cancer. Therefore, the results of this research can show the need for further investigation in subsequent studies.

143. Comparison of OKULIX ray-tracing software with SRK-T and Hoffer-Q formula in intraocular lens power calculation

Purpose: To compare the performance of OKULIX ray-tracing software with SRK-T and Hoffer Q formula in intraocular lens (IOL) power calculation in patients presenting with cataract. Methods: In this prospective study, 104 eyes of 104 patients with cataract who underwent phacoemulsification and IOL implantation were recruited. Three IOL brands were used and for all eyes, IOL power calculation was performed using SRK-T, Hoffer Q formula and also OKULIX ray-tracing software. For all patients, axial length and keratometry data was obtained with IOLMaster 500 device and IOL power was determined using Hoffer Q and SRK-T formula. The IOL powers were also calculated using the OKULIX ray-tracing software combined with CASIA AS-OCT and IOLMaster 500 device. Optically measured axial length of eyes were inserted to OKULIX software from IOLMaster 500 device, and anterior and posterior tomographic and corneal pachymetry data was imported from CASIA AS-OCT into the OKULIX. The performance of each calculation methods was measured by subtracting the predicted postoperative refraction from the postoperative manifest refraction spherical equivalent (MRSE). For each of the 3 methods, the mean absolute prediction error was determined, too. Results: The mean value absolute prediction error by OKULIX, SRK-T and Hoffer Q formulas, respectively, were 0.42 (±0.03), 0.36 (±0.02) and 0.37 (±0.02). The mean absolute prediction error by OKULIX had no significant difference between three IOL groups (P = 0.96), and it was confirmed that there was no meaningful statistically difference in mean absolute prediction error between the OKULIX, SRK-T and Hoffer Q formula. (P = 0.25). Also in each group of implanted IOLs, all three formulas worked with the same accuracy. The prediction error using OKULIX were within ±0.50 diopter in 63.5% of eyes and within ±1.00 diopter in 94.2% of eyes. Conclusion: OKULIX ray-tracing IOL power measurements provides reliable and satisfactory postoperative results, which are comparable to other 3rd generation formulas of SRK-T and Hoffer Q.

ABSTRACT Background and Objectives: Acinetobacter species are responsible for a wide range of clinical complications in hospitalized patients. Antimicrobial treatment of clinical strains of Acinetobacter baumannii may be compromised due to multiple-drug resistance to b-lactams. Aim of this study was to determine antibiotic resistance patterns and frequency of PER and VEB genes in A. baumannii isolates from hospitalized patients. Methods: In this cross-sectional study, 100 clinical strains of A. baumannii were isolated from patients hospitalized in Qom (Iran) using specific culture media and biochemical tests. The disk diffusion method was performed to determine resistance to some antibiotics. Minimum inhibitory concentration (MIC) for cefepime and ceftazidime was evaluated. Identification of ESBL-producing strains and presence of the PER and VEB genes were determined by combined disk test and polymerase chain reaction, respectively. Results: The isolates were highly resistant against cefixime, ceftriaxone and cefepime. Lowest level of resistance was against polymyxin B. In addition, 70% of the isolates were multi-drug resistant. In this study, optimisation of the photocatalytic behaviour of crystal violet (CV) by thiourea (Tu)-codoped TiO2 thin film in fixed bed photoreactor was investigated by central composite designs (CCDs). The effective variables were pH, the concentration of CV dye, flow rate and reaction time. The results of the CCD model showed a good agreement with experimental results, with R2 = 0.9680 (p < 0.0001) and maximum degradation efficiency was obtained at the optimum conditions: dye concentration 8.5 mg/L, pH 9, flow rate 6 mL/min and reaction time 80 min. Subsequently, three absorbing chemical compounds presented in the degradation reaction were obtained by using singular value decomposition (SVD) method and evolving factor analysis (EFA). Then a multivariate curve resolution with alternating least squares (MCR-ALS) was performed to achieve the concentration and spectral profiles for each component. Finally, a hard modelling method was applied to determine the kinetic constants of distinct reactions occurred in the photocatalytic degradation process. The reaction rate constants were calculated for the first and second steps as k1 = 0.08327 (SD = ±0.0015) /min and k2 = 0.045 (SD = ±0.0006)/min, respectively.

The aim of this study was to investigate the effects of natural weathering induced by critical climate conditions on quality of sugarcane bagasse lignocellulosic material in Pars factory. The experimental samples selected from fresh and also stored bagasse for durations of 6 months, 1, 2 and 5 years, were used. The mechanical and the chemical properties as well the color variations were measured on taken samples. The results showed that the long storage durations of bagasse significantly decreased the quality of row materials observed in hand sheet tensile and fold strengths. The results illustrated that the amount of Lignin and extractives increased (44 % in lignin and 32% in extractives amount respectively) and also decrease in amount of cellulose and hemicellulose was found (10% for cellulose and 43% for hemicellulose) during 5 years storage in yard. According to the results, there was an obvious difference in L and a color index of 5 years old bagasse in comparing with those of fresh bagasse. Finally it can be drawn that the use of discorded and weathered bagasse could cause the loss in the final products quality which means more chemical treatment for bleaching and more using long fiber in paper making process.

- Abstract
Understanding the health consequences associated with exposure to risk factors is necessary to inform public health policy and practice. To systematically quantify the contributions of risk factor exposures to specific health outcomes, the Global Burden of Diseases, Injuries, and Risk Factors Study (GBD) 2021 aims to provide comprehensive estimates of exposure levels, relative health risks, and attributable burden of disease for 88 risk factors in 204 countries and territories and 811 subnational locations, from 1990 to 2021.

- Abstract
Detailed, comprehensive, and timely reporting on population health by underlying causes of disability and premature death is crucial to understanding and responding to complex patterns of disease and injury burden over time and across age groups, sexes, and locations. The availability of disease burden estimates can promote evidence-based interventions that enable public health researchers, policy makers, and other professionals to implement strategies that can mitigate diseases. It can also facilitate more rigorous monitoring of progress towards national and international health targets, such as the Sustainable Development Goals. For three decades, the Global Burden of Diseases, Injuries, and Risk Factors Study (GBD) has filled that need. A global network of collaborators contributed to the production of GBD 2021 by providing, reviewing, and analysing all available data. GBD estimates are updated routinely with additional data and refined analytical methods. GBD 2021 presents, for the first time, estimates of health loss due to the COVID-19 pandemic.

- Abstract
Future trends in disease burden and drivers of health are of great interest to policy makers and the public at large. This information can be used for policy and long-term health investment, planning, and prioritisation. We have expanded and improved upon previous forecasts produced as part of the Global Burden of Diseases, Injuries, and Risk Factors Study (GBD) and provide a reference forecast (the most likely future), and alternative scenarios assessing disease burden trajectories if selected sets of risk factors were eliminated from current levels by 2050.

- Abstract
Accurate assessments of current and future fertility—including overall trends and changing population age structures across countries and regions—are essential to help plan for the profound social, economic, environmental, and geopolitical challenges that these changes will bring. Estimates and projections of fertility are necessary to inform policies involving resource and health-care needs, labour supply, education, gender equality, and family planning and support. The Global Burden of Diseases, Injuries, and Risk Factors Study (GBD) 2021 produced up-to-date and comprehensive demographic assessments of key fertility indicators at global, regional, and national levels from 1950 to 2021 and forecast fertility metrics to 2100 based on a reference scenario and key policy-dependent alternative scenarios.
