Daily coronal synoptic observations with the ultraviolet coronagraph spectrometer (UVCS) on SOHO are used to create Carrington maps of line of sight intensities and spectral line widths for H I Lyman alpha and the O 6 1032/1037 doublet. These 2D maps can then be used as inputs to a tomographic inversion routine to produce 3D models of the corona out to the 2.5 solar radius in the polar regions (3.0 solar radius in equatorial regions). The initial results for the determination of O(5+) outflow velocities as a function of heliographic latitude is presented.

The intensity ratio of the O VI doublet in the extended area is analyzed. The O VI intensity data were obtained with the ultraviolet coronagraph spectrometer (UVCS) during the SOHO campaign 'whole sun month'. The long term observations above the north pole of the sun were used for the polar coronal data. Using these measurements, the solar wind outflow velocity in the extended corona was determined. The 100 km/s level is running along the streamer borders. The acceleration of the solar wind is found to be high in regions between streamers. In the central part of streamers, the outflow velocity of the coronal plasma remains below 100 km/s at least within 3.8 solar radii. The regions at the north and south poles, characterized by a more rapid acceleration of the solar wind, correspond to regions where the UVCS observes enhanced O VI line broadenings.

Solar Physics - Abstract
The observations carried out with the ultraviolet coronagraph spectrometer onboard the Solar and Heliospheric Observatory (SOHO) are discussed. The purpose of the observations was to determine the line of sight and radial velocity fields in coronal regions with different magnetic topology. The results showed that the regions where the high speed solar wind flows along open field lines are characterized by O VI 1032 and HI Lyman alpha 1216 lines. The global coronal maps of the line of sight velocity were reconstructed. The corona height, where the solar wind reaches 100 km/s, was determined.

Solar Physics - Abstract
The observations of the ultraviolet coronagraph spectrometer (UVCS), operating onboard the Solar and Heliospheric Observatory (SOHO) spacecraft, are discussed. The purpose of the UVCS is the study of the quiescent coronal streamer and the slow solar wind. The observations started in January 1996. Polarized radiance data in the visible continuum were obtained. Some characteristics of the coronal streamer from the UVCS recorded data are discussed. A model for the source of the slow solar wind in the inner corona is proposed.

The spatial distribution of plasma parameters in the extended corona, derived from the ultraviolet coronagraph spectrometer (UVCS) onboard the Solar and Heliospheric Observatory (SOHO), was investigated. The observations were carried out during the SOHO whole month campaign. Daily coronal scans in the H I Lyman alpha and O VI lambda-lambda 1032 A and 1037 A were used. Maps of outflow velocities of O(5+), based on Doppler dimming of the O VI lines, are discussed. The velocity distribution widths of O(5+) are shown to be a clear signature of coronal holes while the velocity distributions for H(0) show a much smaller effect. The possible physical explanations for some of the observed features are discussed.

The observations of the polar coronal hole regions, obtained with the ultraviolet coronagraph spectrometer (UVCS) onboard the Solar and Heliospheric Observatory (SOHO), are reported on. The O VI 1032 line profiles were found to be narrower within plumes. The difference in the line width increased with the distance from the center of the sun. The analysis of the O VI 1032, H I Lyman alpha 1216 line profiles, and the O VI 1032, 1037 doublet intensity ratio is summarized. The analysis showed that the O VI line width is enhanced in interplume regions, and increases with altitude both in plume and interplume regions. The H I Lyman alpha profiles are wider in the interplume regions. From the doublet intensity ratio data, it is shown that during the minimum of the solar cycle, the acceleration of the solar wind in polar regions reaches approximately 300 km/s at 3.5 solar radii.

The ultraviolet coronal spectrometer (UVCS)/SOHO instrument will provide spectroscopic determinations of plasma parameters describing the evolution of coronal conditions at the site of coronal mass ejections (CME's). Prior to a CME event, UVCS can provide an empirical model of the coronal region where the event originates. The model would include values for the proton density, temperature, and outflow velocity; the electron density and temperature; and minor ion densities, temperatures, and outflow velocities. This information would be determined for heliocentric heights from about 1.5 to 3.5 of the solar radius with a subset of the parameters determined up to heights of 12 of the solar radius. During the event, UVCS would repeatedly sample one or more heights between 1.5 and 3.5 of the solar radius with a time resolution of about 1 min. The goal would be to determine the mass and energy content of the CME as it moves outward. After the event, the conditions in the region would be determined as before. Information about the instrument optical specifications and sensitivity is provided.

START-Register - Survey on anTicoagulated pAtients RegisTer - is an independent, inception-cohort, observational, collaborative database aimed at recording prospectively the clinical history of adult patients starting anticoagulant treatment for any reason and using whatever drug. In this article we present the START-Register and give cross section baseline data focusing on non valvular atrial fibrillation (NVAF). Participants are asked to insert prospectively consecutive patients recorded as electronic file on the web-site of the registry. Required data are: demographic and clinical characteristics of patients, associated risk factors for stroke and bleeding, laboratory routine data, clinical indication for treatment, expected therapeutic range (in cases of treatment with vitamin K antagonists -VKAs). The follow-up is carried out to record: quality of treatment (for patients on VKAs), bleeding complications, thrombotic events, and the onset of any type of associated disease. To date 5252 patients have been enrolled; 97.6% were on VKAs because direct oral anticoagulants (DOAC) have been available in Italy only recently. The median age was 74 years [interquartile range (IQR) 64-80]; males 53.7%. This analysis is focused on the 3209 (61.1%) NVAF patients. Mean CHADS2 score was 2.1±1.1, CHADSVASc score was 3.1±1.3;median age was 76 years (IQR 70-81); 168 patients (5.3%) had severe renal failure [Creatinine clearance (CrCl) 80 years with high prevalence of renal failure.

During most of solar cycle 21 the large-scale photospheric field rotated more rapidly in the Northern Hemisphere than in the southern. The large-scale northern field rotated with a 26.9 day period (synodic), was centered at 15 degrees N, and covered a latitude zone about 24 degrees wide. The large-scale southern field rotated with a periodicity of 28.1 days, was centered at 26 degrees S, and covered a latitude zone about 32 degrees wide. Our analysis showed rotational power at only a few discrete latitudes and frequencies in each hemisphere. The center of each peak lies near to the sunspot differential rotation curve. The largest scale field contributes to the configuration of the coronal and interplanetary magnetic field (IMF). The strength of the first harmonic of the northern field suggests that this structure may be related to the 4-sector pattern observed in the IMF polarity. The southern field had much lower power at the first harmonic of the solar rotation rate and so would contribute only to a 2-sector structure in the IMF. These results were discovered in Fourier analysis of photospheric synoptic charts obtained at the Wilcox Solar Observatory from 1976 to 1986 and confirmed in higher resolution maps from the National Solar Observatory. Mt. Wilson magnetic field measurements from solar cycle 20 show a similar north-south asymmetry.

- Abstract
The optimal duration of anticoagulation in patients with venous thromboembolism (VTE) is uncertain. We investigated whether persistently negative D-dimers in patients with vein recanalization or stable thrombotic burden can identify subjects at low recurrence risk. Outpatients with a first VTE (unprovoked or associated with weak risk factors) were eligible after at least 3 months (12 in those with residual thrombosis) of anticoagulation. They received serial D-dimer measurements using commercial assays with predefined age/sex-specific cutoffs and were followed for up to 2 years. Of 1010 patients, anticoagulation was stopped in 528 (52.3%) with persistently negative D-dimer who subsequently experienced 25 recurrences (3.0% pt-y; 95% confidence interval [CI], 2.0-4.4%). Of the remaining 482 patients, 373 resumed anticoagulation and 109 refused it. Recurrent VTE developed in 15 patients (8.8% pt-y; 95% CI, 5.0-14.1) of the latter group and in 4 of the former (0.7% pt-y; 95% CI, 0.2-1.7; hazard ratio 5 2.92; 95% CI, 1.87-9.72; P 5 .0006). Major bleeding occurred in 14 patients (2.3% pt-y;95%CI, 1.3-3.9)whoresumedanticoagulation. Serial D-dimer measurement is suitable in clinical practice for the identification of VTE patients in whom anticoagulation can be safely discontinued. Essentials Predicting recurrences may guide therapy after unprovoked venous thromboembolism (VTE). We evaluated the DASH score in 827 patients with unprovoked VTE to verify prediction accuracy. A DASH score ≤ 1 had a cumulative recurrence risk at 1 year of 3.6%, as predicted by the model. The DASH score performed better in younger (< 65 years old) subjects. Summary: Background The DASH prediction model has been proposed as a guide to identify patients at low risk of recurrence of venous thromboembolism (VTE), but has never been validated in an independent cohort. Aims To validate the calibration and discrimination of the DASH prediction model, and to evaluate the DASH score in a predefined patient subgroup aged > 65 years. Methods Patients with a proximal unprovoked deep vein thrombosis (DVT) or pulmonary embolism (PE) who received a full course of vitamin K antagonist or direct oral anticoagulant (> 3 months) and had D-dimer measured after treatment withdrawal were eligible. The DASH score was computed on the basis of the D-dimer level after therapy withdrawal and personal characteristics at the time of the event. Recurrent VTE events were symptomatic proximal or distal DVT/PE, and were analyzed with a time-dependent analysis. Observed 12-month and 24-month recurrence rates were compared with recurrence rates predicted by the DASH model. Results We analyzed a total of 827 patients, of whom 100 (12.1%) had an objectively documented recurrence. As compared with the original DASH cohort, there was a greater proportion of subjects with a ‘low-risk’ (≤ 1) DASH score (66.3% versus 51.6%, P < 0.001). The slope of the observed versus expected cumulative incidence at 2 years was 0.71 (95% confidence interval 0.51–1.45). The c-statistic was lower for subjects aged > 65 years (0.54) than for younger subjects (0.72). Introduction In addition to systemic hemodynamics, the management\ud of neurocritically ill patients is often informed by neuromonitoring. In\ud the absence of high-level evidence clinicians are often guided by personal\ud and local expertise. Little is known about practices as they pertain\ud to the use of such monitoring in patients with acute brain injury (ABI).\ud Objectives To investigate practices in bedside monitoring for ABI patients.\ud Particularly interested in differences among “neurointensivists”\ud (NIs; defined here as intensivists whose clinical practice is comprised\ud > 1/3 by neurocritical care) and other intensivists (OIs). Also, to\ud explore patterns specific to traumatic brain injury (TBI) and subarachnoid\ud hemorrhage (SAH), as well as preferences and availability of\ud particular technologies/devices.\ud Methods Electronic survey of 22 items including two case-based scenarios;\ud endorsed by SCCM (9,000 recipients) and ESICM (on-line\ud newsletter) in 2013. A sample size of 370 was calculated based on a\ud population of 10,000 physician members, a 5 % margin error, and\ud 95 % confidence interval. We summarized results using descriptive\ud statistics (proportions with 95 % confidence intervals). A chi-square\ud test was used to compare proportions of responses between NIs and\ud OIs with a significance p < 0.05.\ud Results There were 655 responders (66 % completion rate); 422(65 %)\ud were classified as OIs and 226(35 %) as NIs. More NIs follow\ud hemodynamic protocols for neurocritically-ill patients (56 % vs. 43 %, p\ud 0.001), in TBI (44.5 % vs. 33.3 %, p 0.007), and in SAH (38.1 % vs. 21.3 %,\ud p < 000.1). For delayed cerebral ischemia (DCI), more NIs target cardiac\ud index (CI) (35 % vs. 21 %, p 0.0001), and fluid responsiveness (62 % vs.\ud 53 %, p 0.03), use more bedside ultrasound (BUS) (42 % vs. 29 %, p\ud 0.005) and arterial waveform analysis (40 % vs. 29 %, p 0.02). For DCI\ud neuromonitoring, NIs use more angiography (57 % vs. 43 %, p 0.004),\ud TCD (46 % vs. 38 %, p 0.0001), and CTP (32 % vs.16 %, p 0.0001). For\ud CPP optimization in TBI, NIs use more arterial waveform analysis (45 %\ud vs. 35 %, p 0.019), and BUS (37 % vs. 27.7 %, p 0.023), while more OIs\ud monitor mixed venous oxygen saturation (54.1 % vs. 45 %, p 0.045). For\ud TBI neuromonitoring, NIs use more PbtO2 (28 % vs. 10 %, p 0.0001). In\ud the case scenario of raised ICP/low PbtO2, most employ analgosedation\ud (47 %) and osmotherapy (38 %). Fewer make use of preserved pressure\ud reactivity, particularly OIs (vasopressor use 23 % vs. 34 %, p 0.014).\ud Conclusions There is large heterogeneity in the use of monitoring protocols,\ud variables, and technologies/devices. “Neurointensivists” not only\ud employ more neuromonitoring but also more hemodynamic monitoring\ud in patients with acute brain injury. Soft X-ray observations of the limb event on 1980 April 30 are summarized. These consist of maps made with the Flat Crystal Spectrometer and calcium and iron spectra obtained with the Bent Crystal Spectrometer. The physical conditions, e.g., temperature, density, and energy fluxes, are estimated. The conductive losses exceed the radiative flux during the flare by a factor of about 100. Spectral lines were observed to have enhanced broadening, probably indicating turbulence, for several minutes, coincident with the hard X-ray burst. Since the estimated cooling time is less than the duration of the hot plasma, continuous heating is likely. An estimate is derived for the energy required of 3 x 10/sup 30/ ergs.

Preliminary results obtained for three solar flares with the bent crystal spectrometer on the SMM are presented. Resonance and satellite lines of Ca XIX and XVIII and Fe XXV and XXIV are observed together with the Fe XXVI Lyman-alpha line. Plasma properties are deduced from line ratios and evidence is presented for changes of line widths coincident with the occurrence of a hard X-ray impulsive burst. Fe K-alpha spectra from a disk center and a limb flare agree with the predictions of a fluorescence excitation model. However, a transient Fe K-alpha burst observed in a third flare may be explained by the collisional ionization of cool iron by energetic electrons.

The optimal duration of anticoagulation in patients with venous thromboembolism (VTE) is uncertain. We investigated whether persistently negative D-dimers in patients with vein recanalization or stable thrombotic burden can identify subjects at low recurrence risk. Outpatients with a first VTE (unprovoked or associated with weak risk factors) were eligible after at least 3 months (12 in those with residual thrombosis) of anticoagulation. They received serial D-dimer measurements using commercial assays with predefined age/sex-specific cutoffs and were followed for up to 2 years. Of 1010 patients, anticoagulation was stopped in 528 (52.3%) with persistently negative D-dimer who subsequently experienced 25 recurrences (3.0% pt-y; 95% confidence interval [CI], 2.0-4.4%). Of the remaining 482 patients, 373 resumed anticoagulation and 109 refused it. Recurrent VTE developed in 15 patients (8.8% pt-y; 95% CI, 5.0-14.1) of the latter group and in 4 of the former (0.7% pt-y; 95% CI, 0.2-1.7; hazard ratio = 2.92; 95% CI, 1.87-9.72; P = .0006). Major bleeding occurred in 14 patients (2.3% pt-y; 95% CI, 1.3-3.9) who resumed anticoagulation. Serial D-dimer measurement is suitable in clinical practice for the identification of VTE patients in whom anticoagulation can be safely discontinued. Unmanned aerial vehicles (UAVs) have recently received great interest due to their great potential in both military and civil applications [1]-[4]. Anyway, testing and construction of UAVs is expensive and time consuming. In this context, the simulation approach can help to reduce the cost both design and tests. In this paper, the authors used COMSOL Multiphysics 5.1 to design a UAV's pod equipped with a Solid Oxide Fuel Cell (SOFCs). The study was developed in the framework of the European project SUAV (Microtubular Solid Oxide Fuel Cell Power System development and integration into a Mini-UAV). The stack's maximum power is 200 W and is fed by propane. Thanks to his energy density, using propane as fuel permits to store more energy than fuel cell fed by hydrogen. The aim of study was to calculate the temperature distributions inside the pod having information about the air intakes' size. An simple experimental test was carried out to validate the simulation. The test's set up consists of a PVC tube where the SOFC stack is located inside as showed in figure 1. In front of PVC tube was located a mask with variable surface area (from 5% to 75% open). Experimental test had permitted to evaluate the influence of outlet size on temperature distribution. The inlet air was controlled through a mass flow rate according to the internal temperature and pressure transmitters were used to measure the pressure drop along the tube. In early study, COMSOL Multiphysics simulation will be validate through experimental data and will used to evaluate new air intake configuration (different position and size) as showed in fig.2 . In the following figure, the geometry implemented in COMSOL and preliminary fluid dynamic results are reported. A turbulent approach was used to solve the problem. Early results showed that pressure drop and velocity field are in accordance with experimental tests. The next steps will be the implementation of model with "heat transfer in fluid" physic to know the distribution of temperature in the domain for a complete model validation model. The magnetic field shapes the structure of the solar corona, but we still know little about the interrelationships between the coronal magnetic field configurations and the resulting quasi-stationary structures observed in coronagraphic images (such as streamers, plumes, and coronal holes). One way to obtain information on the large-scale structure of the coronal magnetic field is to extrapolate it from photospheric data and compare the results with coronagraphic images. Our aim is to verify whether this comparison can be a fast method to systematically determine the reliability of the many methods that are available for modeling the coronal magnetic field. Coronal fields are usually extrapolated from photospheric measurements that are typically obtained in a region close to the central meridian on the solar disk and are then compared with coronagraphic images at the limbs, acquired at least seven days before or after to account for solar rotation. This implicitly assumes that no significant changes occurred in the corona during that period. In this work, we combine images from three coronagraphs (SOHO/LASCO-C2 and the two STEREO/SECCHI-COR1) that observe the Sun from different viewing angles to build Carrington maps that cover the entire corona to reduce the effect of temporal evolution to about five days. We then compare the position of the observed streamers in these Carrington maps with that of the neutral lines obtained from four different magnetic field extrapolations to evaluate the performances of the latter in the solar corona. Our results show that the location of coronal streamers can provide important indications to distinguish between different magnetic field extrapolations. [ABSTRACT FROM AUTHOR]

The efficacy of adjusted-dose oral anticoagulant therapy (OAT) in the prevention of thrombotic complications in various clinical conditions is well documented. Management of OAT requires a trained physician, an organized system Of follow-up, reliable international normalized ratio monitoring, and good patient communication and education. Drug interactions with coumarins are a major cause of excessive anti coagulation and hence could be an important determinant of bleeding in patients on OAT. An analysis conducted in Toscana in 2005 found 31,221 patients persistently on OAT. During the same period, potential drug-drug interactions were detected in 11,778 of these patients (37.7%). Thus, the regional government enacted a specific law for the organization of OAT management to give all patients a minimal standard of quality of OAT. A specific educational campaign was promoted with the slogan "Written is better." Three possible models to follow individual patients were identified in relation to available resources and geographic characteristics of the living area: Anticoagulation Clinic (AC), General Practitioner (GP), or combined AC and GP management. This experience, although obtained in a limited geographic area, could help improve the efficacy and safety of OAT management.

The use of corticosteroids in patients with septic shock remains controversial. Questions remain regarding the more appropriate dose, the optimal timing to initiate therapy, the selection of patients who will benefit most from the treatment and the exact mechanisms involved in their effectiveness. Recent studies have highlighted that, in critically ill patients, corticosteroid metabolism was reduced and associated with high circulating cortisol levels. Hence the required doses of hydrocortisone may be lower than the currently recommended doses in septic shock (i.e. 200 mg/day). However, altered expression and/or function of corticosteroid receptors may still suggest that higher hydrocortisone doses are necessary to overcome this so-called "steroid-resistance". In this article, we summarized these recent concepts and discussed how they could influence the administration of corticosteroids in such patients.
